Eine Dysbalance zwischen regulatorischen und proinflammatorischen T-Helferzellen kann zu Autoimmunerkrankungen führen. In dieser methodischen Arbeit wurde die Polarisierbarkeit von peripheren T-Lymphozyten durch verschiedene Zytokin-Stimuli untersucht. Hauptziel war es, CD4+CD25-CD127- Lymphozyten durch Stimulation mit einem IL-2 und TGFβ-beinhaltenden Zytokin-Cocktail (Treg-Cocktail) zu iTregs zu polarisieren und deren Suppressionsfunktion auf autologe Effektor-Leukozyten zu untersuchen.
Es erfolgte eine Phänotypisierung der PBMCs gesunder Probanden, insbesondere im Hinblick auf die Verteilung der T-Lymphozyten-Subpopulationen, deren Zytokinproduktion und FoxP3-Expression. Zudem wurden aus den PBMCs der Probanden Tregs (CD4+CD25+CD127low/-) sowie CD4+CD25-CD127- Zellen isoliert und deren Funktionsfähigkeit durch die Untersuchung ihrer Suppressionsfunktion auf autologe Effektor-Lymphozyten analysiert. Die Zellen wurden mittels verschiedener Zytokin-Cocktails in Richtung Treg sowie in Richtung Th17-Zellen polarisiert; anschließend wurde die Funktionsfähigkeit der polarisierten Zellen in Suppression-Assays gemessen.
Wir konnten zeigen, dass die CD4+CD25+CD127low/- Zellen Tregs mit der Fähigkeit zur Suppression der Proliferation autologer Effektor-Lymphozyten waren. Bei den CD4+CD25-CD127-Zellen handelte es sich um T-Lymphozyten ohne Suppressionsfunktion. Nach Stimulation der CD4+CD25-CD127-Zellen mit dem Treg-Cocktail zeigten die Zellen eine mit den Tregs vergleichbare Suppressionsfunktion.
Mit dieser Studie haben wir eine aktuelle methodische Quelle für die Untersuchung von Phänotyp und Funktion regulatorischer T-Zellen sowie für die Stimulation peripherer T-Lymphozyten hin zu Tregs geschaffen, die als Basis für Folgeversuche dienen soll, in denen Zellen von Patienten mit Autoimmunkrankheiten untersucht werden sollen. Da sich die Inflammation bei Autoimmunerkrankungen insbesondere in den betroffenen Geweben abspielt, wäre eine Studie anzustreben, in der aus dem Blut isolierte T-Lymphozyten den Zellen aus den entzündeten Geweben gegenübergestellt werden. Ergänzend sollte eine Phänotypisierung der Tregs und der CD4+CD25-CD127- Zellen nach der Zytokin-Stimulation erfolgen.
Zusammenfassend konnte die Plastizität peripherer T-Lymphozyten in Richtung Treg gezeigt werden. Besonders hervorzuheben ist die bislang wenig untersuchte Zellpopulation der CD4+CD25-CD127- Zellen, die eine vielversprechende Zellpopulation für die in vitro Induktion von Tregs darstellt.
Ausgehend von einer potentiell anti-Tumor-aktiven B-Zellen des menschlichen Immunsystem haben sich durch die Untersuchung des peripheren Blutes auf das Vorliegen von lambda3r-positiven, CD19+B-Zellen bei Magenkarzinompatienten und Probanden unterschiedlichen Alters einige sehr interessante Ergebnisse im Bereich der B-Zellimmunität ergeben. Es scheint dabei eine Art B-Zell-Immunosurveillance in Form dieser B-Zellen, sowohl bei Karzinompatienten, als auch bei Gesunden von früher Kindheit an zu geben. Die relative Verteilung dieser Zellen ändert sich dabei im Laufe des Lebens ensprechend den Veränderungen des gesamten B-Zellkompartiments. Es findet eine Abnahme mit dem Alter statt. Im Falle des Auftretens eines Magenkarzinoms kommt es dann zu einer relativen Expansion der in dieser Arbeit beschriebenen lambda3r-positiven CD19+B-Zellen trotz einer gleichzeitig stattfindenden bisher nicht erklärlichen Involution des restlichen B-Zellsystems. Bei der relativen Zunahme dieser Zellen handelt es sich um eine Art Boosterung. Das expandierte Zellkompartiment zeigt dabei Reifungstendenzen, sichtbar im Verlust des Oberflächenmoleküls IgD sowie der Expression von CD27 und IgG. Dem Oberflächenmarker CD5 scheint im Gegensatz zur initialen Hypothese bei der Erst-beschreibung der SC-1-positiven B-Zelle keine zentrale Rolle zuzukommen.
Background: There is much evidence that T cells are strongly involved in the pathogenesis of localized and systemic forms of scleroderma (SSc). A dysbalance between FoxP3+ regulatory CD4+ T cells (Tregs) and inflammatory T-helper (Th) 17 cells has been suggested. Methods: The study aimed (1) to investigate the phenotypical and functional characteristics of Th17 and Tregs in SSc patients depending on disease manifestation (limited vs. diffuse cutaneous SSc, dcSSc) and activity, and (2) the transcriptional level and methylation status of Th17- and Treg-specific transcription factors. Results: There was a concurrent accumulation of circulating peripheral IL-17-producing CCR6+ Th cells and FoxP3+ Tregs in patients with dcSSc. At the transcriptional level, Th17- and Treg-associated transcription factors were elevated in SSc. A strong association with high circulating Th17 and Tregs was seen with early, active, and severe disease presentation. However, a diminished suppressive function on autologous lymphocytes was found in SSc-derived Tregs. Significant relative hypermethylation was seen at the gene level for RORC1 and RORC2 in SSc, particularly in patients with high inflammatory activity. Conclusions: Besides the high transcriptional activity of T cells, attributed to Treg or Th17 phenotype, in active SSc disease, Tregs may be insufficient to produce high amounts of IL-10 or to control proliferative activity of effector T cells in SSc. Our results suggest a high plasticity of Tregs strongly associated with the Th17 phenotype. Future directions may focus on enhancing Treg functions and stabilization of the Treg phenotype.
Introduction
Juvenile idiopathic arthritis is a heterogeneous T cell-mediated autoimmune disease with symptoms of premature aging of the immune system (immunosenescence). The present work is an investigation of immunosenescence parameters, such as quantity of naive and CD28- T cells, T cell receptor excision circles, relative telomere length and alterations of peripheral T cell replication, and was performed via comparison of a case of acute exacerbation of juvenile idiopathic arthritis against six patients with juvenile idiopathic arthritis with disease remission and six age-matched healthy donors over a follow-up course of 12 months.
Case presentation
Phenotypical T cell characterization and intracellular interferon γ, tumor necrosis factor α, and interleukin 2 production were studied in peripheral blood mononuclear cells from seven patients with juvenile idiopathic arthritis and six healthy control donors, with findings determined by flow cytometry. T cell receptor excision circles and relative telomere length quantification were performed on deoxyribonucleic acid isolated from naive (CD4+CD28+CD45RA+) T cells and investigated via reverse transcription polymerase chain reaction. Ki67 expression was studied by immunohistochemistry on naive T cells. The non-parametric Mann-Whitney U test and Wilcoxon test for two independent groups of variables were used to compare healthy donors with patients with juvenile idiopathic arthritis. During follow-up, patients with juvenile idiopathic arthritis showed lower total counts of naive and CD28-expressing T cells compared to healthy donors. Acute exacerbation led to low naive and CD28+ T cell populations and elevated proportions of Ki67-expressing CD4+ naive T cells. In conditions of exacerbation, T cell receptor excision circle numbers were in the lower range in patients with juvenile idiopathic arthritis and increased after follow-up. Healthy donors showed significantly higher relative telomere lengths compared to patients with juvenile idiopathic arthritis.
Conclusions
This investigation illustrates that the changes in T cell homeostasis in patients with juvenile idiopathic arthritis may be the result of several mechanisms, such as diminished thymus function and peripheral exertions to maintain the peripheral T cell pool. The results also demonstrate that hallmarks of immunosenescence such as decreased naive T cell levels and lower T cell receptor excision circle numbers can only be interpreted together with replication markers such as relative telomere length or Ki67 expression.
Background
Children with different underlying malignant diseases require long-term central venous access. As for port systems in a pectoral position, peripherally implanted port systems in the forearm revealed high levels of technical and clinical success in adult cohorts.
Objective
To investigate the technical and clinical outcomes of percutaneous central venous port implantation in the forearm in adolescents.
Materials and methods
Between April 2010 and August 2020, 32 children ages 9 to 17 years with underlying malignancy received 35 totally implantable venous access ports (TIVAPs) in the forearm. All venous port systems were peripherally inserted under ultrasound guidance. Correct catheter placement was controlled by fluoroscopy. As primary endpoints, the technical success, rate of complications and catheter maintenance were analyzed. Secondary endpoints were the side of implantation, vein of catheter access, laboratory results on the day of the procedure, procedural radiation exposure, amount of contrast agent and reasons for port device removal.
Results
Percutaneous TIVAP placement under sonographic guidance was technically successful in 34 of 35 procedures (97.1%). Procedure-related complications did not occur. During the follow-up, 13,684 catheter days were analyzed, revealing 11 complications (0.8 per 1,000 catheter-duration days), Of these 11 complications, 7 were major and 10 occurred late. In seven cases, the port device had to be removed; removal-related complications did not occur.
Conclusion
Peripheral TIVAP placement in the forearms of children is a feasible, effective and safe technique with good midterm outcome. As results are comparable with standard access routes, this technique may be offered as an alternative when intermittent venous access is required.
Das Bestreben, den Aufbau, die Funktion sowie die Entwicklung des Immunsystems zu verstehen, steht schon lange Zeit im Zentrum des Interesses vieler Forschungsarbeiten, insbesondere um auf Grundlage der gewonnenen Erkenntnisse neue Behandlungsansätze für immunologisch relevante Krankheitsbilder zu entwickeln.
Stillen könnte ein wichtiger Faktor sein, der bei der Entwicklung und Differenzierung von T-Zell-Subpopulationen und Zytokinmustern im Säuglings- und Kindesalter eine bedeutende Rolle spielt.
Die Zielsetzung der hier vorgelegten Promotionsarbeit war es, den potentiellen Effekt des Faktors Stillen auf die Entwicklung, die Verteilung und die Differenzierung von Zell-populationen sowie die Expression von Zytokinen bei gesunden Kindern zu untersuchen. Dies geschah insbesondere im Hinblick auf einen möglicherweise vorhandenen Shift der relativen Verteilung der TH1- und TH2-Zytokinen, da in retrospektiven Kohortenstudien bereits gezeigt werden konnte, dass gestillte Kinder eine geringere Anfälligkeit gegenüber schwerwiegenden bakteriellen Infektionen (BACHRACH ET AL., 2003) sowie einer verminderten Inzidenz von Autoimmunerkrankungen (KOLETZKO ET AL., 1989; PISACANE ET AL., 1994) aufweisen.
Die Studienkohorte bestand aus 196 gesunden Kindern im Alter zwischen 26 Tagen und 12 Jahren und 352 Tagen. Diese wurde in vier Altersgruppen unterteilt (<1, 1- <3, 3- <6 und 6-<13 Jahre) und mittels eines Fragebogens im Hinblick auf ein möglicherweise vorhandenes Bias bezüglich exogener Einflussfaktoren wie Impfungen, Nikotinexposition (FELESZKO ET AL., 2006) und allergische Erkrankungen in der Familie (HRDÝ ET AL., 2010), die in diesem Zusammenhang diskutiert werden, überprüft. Dabei zeigten sich keine signifikanten Unter-schiede zwischen den Gruppen.
Alle immunologischen Parameter wurden in peripherem, heparinisiertem Blut ermittelt. Zunächst wurde mittels Durchflusszytometrie (FACS) eine Phänotypisierung, anhand von antikörpermarkierten Oberflächenantigenen der T-, B- und NK-Zellpopulationen („Immun-status“), durchgeführt. Des Weiteren wurden die mononuklearen Zellen des peripheren Blutes (PBMC) mittels PMA und Ionomycin stimuliert und die Zytokinsekretion durch Brefeldin blockiert. Durch FACS-Analyse wurde die nach 20-24 stündiger Anregung in der Kultur vorhandene intrazellulärer Zytokinexpression von IL2, IFNγ, TNFα, IL4, IL10, TGFβ und IL17 in den T-Zellpopulationen ermittelt. In einem zweiten Schritt wurde der Quotient aus IFNγ und IL4 berechnet, um das Verhältnis zwischen TH1 und TH2 zu analysieren.
Das Datenmaterial zeigt die Entwicklung der T-Zell-Subpopulationen mit dem Alter. Junge Kinder zeigen eine durch regulatorische T-Zellen (Treg) bzw. TH0-Zellen vorherrschende TGFβ und IL2-Expression, während ältere Kinder das gesamte Repertoire an TH1 (IFNγ, TNFα) und TH2-Zytokinen (IL4), insbesondere durch T-Gedächtniszellen, exprimieren. Diese Ergebnisse bestätigten bereits zuvor beschriebene – vom Faktor Stillen unabhängige – altersabhängige Veränderungen der Zellpopulationen und der Zytokinexpression. Aus diesem Grund konnte von einer „normalen Verteilung“ der Studienkohorte ausgegangen werden.
Zwischen gestillten und nicht-gestillten Kindern hat sich gezeigt, dass sich die Größe und das Verhältnis der übergeordneten Zellpopulationen (T-Helferzellen, zytotoxische T-Zellen) sowie die Reifung der T-Zellen (naive T-Zellen, T-Gedächtniszellen) bezogen auf das Alter nicht unterscheiden. Hingegen zeigt der Faktor Stillen in der Tat einen Einfluss auf die TH1/TH2-Balance. Bei gestillten Kindern lässt sich eine stärkere Gewichtung in Richtung der TH2-Zytokine feststellen (niedrigere IFNγ/IL4-Ratio), so dass davon auszugehen ist, dass Stillen einen so genannten TH2-Shift induziert. Dieses gegenüber nicht-gestillten Kindern „verschobene“ Gleichgewicht bleibt bis zur Altersgruppe der 6-13 Jährigen konstant. Gestillte Kinder haben zudem, im Vergleich zu Formula-ernährten Kindern, ein höheres Vermögen TH1-Zytokine wie TNFα und IFNγ zwischen dem dritten und sechsten Lebensjahr zu bilden.
Diese Daten entsprechen nicht einem vorherrschenden TH2-Muster und einer Allergiedisposition, aber sie können die geringere Inzidenz von bakteriellen Infektionen im Vorschulalter (TH1-Antworten benötigt) und von TH1-vermittelten Autoimmunerkrankungen bei gestillten Kindern erklären.
Aufgrund der Ergebnisse wird deutlich, dass Muttermilch einen bedeutenden Einfluss auf das Immunsystem hat. Insbesondere konnte gezeigt werden, dass der Einfluss der Muttermilchernährung über den eigentlichen Zeitraum des Stillens hinausreicht. Eine Prägung des Immunsystems durch die Muttermilch, während der frühen Kindheit, erscheint deshalb sehr wahrscheinlich. Diese Ergebnisse sollten selbstverständlich durch weiterreichende Studien, die eventuell vorhandene weitere Störgrößen wie genetische Prädispositionen oder auch Umwelttoxine einschließen, verifiziert werden.
Zudem wäre es von Interesse welche in der Muttermilch enthaltene Stoffe zu diesem TH2-Shift beitragen oder ob es sich bei den Einflussfaktoren vielmehr um in der Muttermilch nicht enthaltene Stoffe handelt, die in Formula-Nahrung enthalten sind, wie zum Beispiel Kuhmilchantigene.
Schlussendlich bleibt festzuhalten, dass Muttermilch erwiesene positive Vorteile mit sich bringt, wie geringere Infektionsraten (Atemwegsinfektionen, Otitis media) und eine Risiko-verminderung für bestimmte Erkrankungen (Diabetes mellitus Typ1, Morbus Crohn, Multiple Sklerose) im späteren Leben. Die Ergebnisse dieser Arbeit können bestätigen, dass das Stillen mit Muttermilch tatsächlich einen Einfluss auf die Entwicklung des individuellen Immunsystems hat, der auch nach dem Abstillen weiter anhält.
Eine wichtige Rolle kann diese Erkenntnis bei der Beratung werdender Mütter spielen, gerade in Hinblick auf ein gegebenenfalls erhöhtes endogenes familiäres Risiko für beispielsweise Autoimmunerkrankungen. Folgearbeiten sind sicher wünschenswert, um den pathophysiologischen Hintergrund dieser beobachteten Daten besser zu verstehen.
The aim of this research was to characterize cognitive abilities in patients with Glut1-Deficiency syndrome (Glut1DS) following ketogenic diet therapy (KDT). Methods: The cognitive profiles of eight children were assessed using the Wechsler Intelligence Scale (WISC-IV). The effect of ketogenic diet therapy (KDT) on individual subareas of intelligence was analyzed considering the potential influence of speech motor impairments. Results: Patients with Glut1DS showed a wide range of cognitive performance levels. Some participants showed statistically and clinically significant discrepancies between individual subdomains of intelligence. Both variables, KDT initiation as well as duration, had a positive effect on the overall IQ score. Significant correlations were partially found between the time of KDT initiation and the level of IQ scores, depending on the presence of expressive language test demands of the respective subtests of the WISC-IV. Accordingly, the participants benefited les in the linguistic cognitive domain. The discrepancies in cognitive performance profiles of patients with Glut1DS can be attributed to the possibility of a negative distortion of the results due to the influence of speech motor impairments. Conclusions: The individual access skills of test persons should be more strongly considered in test procedures for the assessment of intelligence to reduce the negative influence of motor deficits on test performance. Specific characterization and systematization of the speech disorder are indispensable for determining the severity of speech motor impairment in Glut1DS. Therefore, a stronger focus on dysarthria during diagnosis and therapy is necessary.
Das Verfahren der intracytoplasmatischen Spermainjektion (ICSI) wird seit 1995 an der Universitäts-Frauenklinik Würzburg angewandt. Diese Studie untersuchte die physische und psychische Entwicklung von 73 ICSI-Kindern sowie die Lebenszufriedenheit ihrer Mütter. Anhand des Enzephalopathie-Fragebogens von Meyer-Probst, des Mann-Zeichen-Tests nach Ziler sowie einiger demographischer Variablen konstatierten wir eine durchschnittliche Entwicklung der ICSI-Kinder. Bei den Risikokindern (Mehrlinge, Frühgeborene und Kinder mit zu geringem Geburtsgewicht) zeichnete sich ein Trend zu schlechteren Ergebnissen ab. Ein signifikanter Unterschied errechnete sich nur zwischen Einlingen und Mehrlingen (Irrtumswahrscheinlichkeit < 5 %). Eine erhöhte Fehlbildungsrate, wie in der Literatur häufig beschrieben, konnten wir nicht verzeichnen. Die Lebenszufriedenheit der ICSI-Mütter entsprach dem Vergleichskollektiv, die höchste Zufriedenheit konstatierten wir bezüglich der Freude mit ihren Kindern. Mehrlingsmütter zeigten sich trotz der erhöhten Belastung nicht unzufriedener. Frauen, die sich mehreren Zyklen unterziehen mussten, bis es zu einer erfolgreichen Schwangerschaft kam, weisen immer noch eine geringere Selbstzufriedenheit auf.
Im Vergleich zu anderen europäischen und nicht-europäischen Ländern steht die pädiatrische Palliativversorgung in Deutschland noch am Anfang. Es besteht weiterhin ein dringlicher Bedarf an Forschung, fehlen Instrumente der Qualitätskontrolle wie Leitlinien und vor allem eine geregelte Finanzierung. Aktuell sind die ersten überregionalen Organisationsstrukturen entstanden, um die Entwicklung auf diesem Gebiet voranzutreiben. Dennoch wird an vielen Kliniken Deutschlands bereits gute und segensreiche Arbeit für sterbende Kinder und ihre Familien geleistet. Nach einer Zusammenfassung von Grundlagen der pädiatrischen Palliativmedizin sowie einer kurzen Darstellung der aktuellen Situation in Deutschland wird die Palliativversorgung an der Universitäts-Kinderklinik in Würzburg beschrieben. Die Ergebnisse eines Pilotprojektes zur häuslichen Palliativversorgung krebskranker Kinder in den Jahren von 2004 bis 2006 zeigen, dass über 85% der Kinder bis zu ihrem Tod zu Hause versorgt werden konnten. Neben der Beschreibung und Diskussion der bisherigen Arbeit im Pilotprojekt werden die Voraussetzungen für die Entwicklung eines spezialisierten pädiatrischen Palliative Care Teams aufgezeigt.
