Der Einfluss von Methylphenidat auf die affektive Bildverarbeitung bei erwachsenen AD(H)S Patienten

Die vorliegende Arbeit wurde im Rahmen des ADHS-Forschungsprojektes der Universität Würzburg zur Identifikation von Endophänotypen der ADHS durchgeführt. Im Fokus des Interesses stand dabei die Untersuchung emotionaler Verarbeitungsprozesse bei erwachsenen ADHS-Patienten. Conzelmann und Kollegen (Conzelmann, et al., 2009) konnten zeigen, dass erwachsene ADHS-Patienten vom Mischtypus und vom hyperaktiv/ impulsiven Typus eine defizitäre affektive Startle Modulation aufweisen. Basierend auf diesen Ergebnissen stellte sich in der vorliegenden Arbeit die Frage, ob dieser defizitäre Schreckreflex auf emotionale Bilder des International Affective Picture Systems (IAPS) zunächst replizierbar und falls ja, durch die Einnahme eines Methylphenidatpräparats (MPH) vermindert oder gänzlich aufgehoben werden kann. Um dieser Frage nachzugehen, wurden zunächst im Rahmen einer Vorstudie 15 erwachsene ADHS-Patienten (Mischtypus) aus der Studie von Conzelmann et al. (2009) nach einer Zeitspanne von einem bis zwei Jahren ein weiteres Mal getestet. Etwa eine Stunde vor der zweiten Testung erhielten die Patienten die Instruktion, ihr MPH-Präparat einzunehmen. Im Anschluss daran wurden den Patienten positive, neutrale und negative IAPS-Bilder präsentiert. Diese Bilddarbietung wurde in unregelmäßigen Abständen von einem akustischen Störgeräusch unterbrochen, den die Patienten möglichst ignorieren sollten. Im zweiten Teil der Untersuchung wurden die Probanden sowohl während der ersten als auch während der zweiten Testung angewiesen, die einzelnen Bilder hinsichtlich ihrer Valenz (angenehm versus unangenehm) und ihres Arousals (aufregend versus ruhig) zu bewerten. Von den 15 getesteten Patienten konnten 13 in die Auswertung mit einbezogen werden (2 Patienten hatten unzureichend auf den Ton reagiert). Unterschiede zwischen den beiden Testzeitpunkten (also ohne und mit MPH) konnten bezüglich der affektiven Startle Modulation lediglich auf Bilder mit hohem Arousal festgestellt werden. Für diese spezifischen Stimuli zeigten die Patienten ohne MPH keine verringerte Startle Reaktion während der Betrachtung positiver Bilder. Mit MPH konnte dieses Ausbleiben der Startle Attenuation aufgehoben werden. Anders als bei der affektiven Startle Modulation, konnte bei den zusätzlich erhobenen physiologischen Maßen weder ein systematischer Einfluss durch die IAPS-Bilder noch durch die Medikation festgestellt werden. Gleiches ließ sich bei den Valenz- und Arousalratings feststellen. Ob sich diese positive MPH-Wirkung auf die affektive Startle Modulation auch unabhängig vom Arousalgehalt der betrachteten Bilder zeigt, sollte im Anschluss an diese Vortestung mittels einer zusätzlichen Studie mit wesentlich größerem Stichprobenumfang untersucht werden. Die Hauptstudie wurde sowohl doppelblind als auch placebo-kontrolliert und im cross-over Design durchgeführt. Die Testprozedur am Computer war vergleichbar mit dem Ablauf der Vortestung. Im Rahmen der Hauptstudie wurden 71 AD(H)S-Patienten (60 vom Mischtypus, 11 vom vorwiegend unaufmerksamen Typus) zweimal getestet. Von diesen 71 Patienten konnten letztlich 61 in die Auswertung der Haupttestung mit einbezogen werden (ein ADHS-Patient (Mischtypus) kam zur zweiten Testung nicht und 9 ADHS-Patienten (Mischtypus) hatten unzureichend auf den Startle-Ton reagiert). Dabei konnte zunächst für die Bilder mit hohem Arousal die defizitäre Startle Modulation auf positive Bilder repliziert werden. Durch die MPH-Einnahme ließ sich dieses Defizit jedoch nicht beheben. Da Conzelmann et al. (2009) lediglich beim Mischtypus und hyperaktiv-impulsiven Typus eine defizitäre Startle Modulation nachweisen konnten, wurde eine vergleichbare Auswertung für die Subgruppe der 51 ADHS-Patienten vom Mischtypus durchgeführt. Dabei unterschieden sich die Ergebnisse dieser Subgruppe im Wesentlichen nicht von der Gesamtstichprobe. Auch bei der Überprüfung der genetischen Hypothesen ließ sich keine Interaktion zwischen der Medikation und dem DRD4-7r, dem COMT Val/Met und dem DAT1-10r Polymorphismus feststellen. Die Ergebnisse zeigen zum einen eine defizitäre Startle Modulation der 61 AD(H)S-Patienten, die vergleichbar ist mit dem gezeigten Defizit von Conzelmann et al. (2009) bei Patienten des Mischtypus. Ein positiver Effekt durch die MPH-Einnahme konnte dabei ebenso wenig bestätigt werden wie eine mögliche Interaktion verschiedener dopaminerger Genpolymorphismen auf die MPH-Wirkung. Zusätzliche Studien werden zeigen, ob diese defizitäre Verarbeitung tatsächlich durch die MPH-Einnahme unbeeinflusst bleibt oder ob letztlich mittels medikamentöser Interventionen doch noch eine entsprechende Symptomverbesserung bei erwachsenen AD(H)S-Patienten zu erzielen ist.
Dysfunktionen des serotonergen Neurotransmittersystems, innerhalb dessen die allelischen Variationen des 5-HTTLPR-Polymorphismus wiederum einen zentrale Rolle einnehmen, werden für die Genese verschiedener psychischer Erkrankungen diskutiert. Untersucht wurde die Interaktion zwischen der allelischen Variationen des 5-HTTLPR-Polymorphismus und Lebensereignissen, die mit Hilfe des Life History Calendar von Caspi bei 123 aADHS-Patienten erfasst wurden. Die Teilnehmer wurden über Lebenserfahrungen bis zu ihrem 21. Lebensjahr genau befragt, die so in additiver Wertung in den Life-Event-Effekt einflossen. Zudem wurden mit Hilfe der Persönlichkeitstests TPQ und NEO-PI-R Punktescores erhoben. Eine Marker*Life Event-Interaktion wurde nachgewiesen. Bei aAHDS-Patienten, die die homozygot lange Variante des 5-HTTLPR-Polymorphismus tragen, ist eine höhere Zahl an erlebten Life Events mit einem größerem Risiko assoziiert, eine Cluster-B-Persönlichkeitsstörung zu entwickeln. Eine geringere Anzahl an Life Events ist assoziiert mit einem geringerem Risiko für Persönlichkeitsstörungen.
Attention-deficit/hyperactivity disorder (ADHD) is a genetically complex childhood onset neurodevelopmental disorder which is highly persistent into adulthood. Several chromo-somal regions associated with this disorder were identified previously in genome-wide linkage scans, association (GWA) and copy number variation (CNV) studies. In this work the results of case-control and family-based association studies using a can-didate gene approach are presented. For this purpose, possible candidate genes for ADHD have been finemapped using mass array-based SNP genotyping. The genes KCNIP4, CDH13 and DIRAS2 have been found to be associated with ADHD and, in addition, with cluster B and cluster C personality disorders (PD) which are known to be related to ADHD. Most of the associations found in this work would not withstand correction for multiple testing. However, a replication in several independent populations has been achieved and in conjunction with previous evidence from linkage, GWA and CNV studies, it is assumed that there are true associations between those genes and ADHD. Further investigation of DIRAS2 by quantitative real-time PCR (qPCR) revealed expression in the hippocampus, cerebral cortex and cerebellum of the human brain and a significant increase in Diras2 expression in the mouse brain during early development. In situ hybrid-izations on murine brain slices confirmed the results gained by qPCR in the human brain. Moreover, Diras2 is expressed in the basolateral amygdala, structures of the olfactory system and several other brain regions which have been implicated in the psychopatholo-gy of ADHD. In conclusion, the results of this work provide further support to the existence of a strong genetic component in the pathophysiology of ADHD and related disorders. KCNIP4, CDH13 and DIRAS2 are promising candidates and need to be further examined to get more knowledge about the neurobiological basis of this common disease. This knowledge is essential for understanding the molecular mechanisms underlying the emergence of this disorder and for the development of new treatment strategies.
The aim of this project was to investigate whether reflex-like innate facial reactions to tastes and odors are altered in patients with eating disorders. Qualitatively different tastes and odors have been found to elicit specific facial expressions in newborns. This specificity in newborns is characterized by positive facial reactions in response to pleasant stimuli and by negative facial reactions in response to unpleasant stimuli. It is, however, unclear, whether these specific facial displays remain stable during ontogeny (1). Despite the fact that several studies had shown that taste-and odor-elicited facial reactions remain quite stable across a human’s life-span, the specificity of research questions, as well as different research methods, allow only limited comparisons between studies. Moreover, the gustofacial response patterns might be altered in pathological eating behavior (2). To date, however, the question of whether dysfunctional eating behavior might alter facial activity in response to tastes and odors has not been addressed. Furthermore, changes in facial activity might be linked to deficient inhibitory facial control (3). To investigate these three research questions, facial reactions in response to tastes and odors were assessed. Facial reactions were analyzed using the Facial Action Coding System (FACS, Ekman & Friesen, 1978; Ekman, Friesen, & Hager, 2002) and electromyography.
The Contribution of Common and Rare Variants to the Complex Genetics of Psychiatric Disorders

Attention deficit/hyperactivity disorder (ADHD), one of the most frequent childhood-onset, chronic and lifelong neurodevelopmental diseases, affects 5 - 10% of school – aged children and adolescents, and 4% of adults. The classified basic symptoms are - according to the diagnostic system DSM-VI - inattentiveness, impulsivity and hyperactivity. Also daily life of patients is impaired by learning problems, relationship crises, conflicts with authority and unemployment, but also comorbidities like sleep - and eating problems, mood - and anxiety disorders, depression and substance abuse disorders are frequently observed. Although several twin and family studies have suggested heritability of ADHD, the likely involvement of multiple genes and environmental factors has hampered the elucidation of its etiology and pathogenesis. Due to the successful medication of ADHD with dopaminergic drugs like methylphenidate, up to now, the search for candidate genes has mainly focused on the dopaminergic and - because of strong interactions - the serotonergic system, including the already analyzed candidate genes DAT1, DRD4 and 5, DBH or 5-HTTLPR. Recently, DNA copy number changes have been implicated in the development of a number of neurodevelopmental diseases and the analysis of chromosomal gains and losses by Array Comparative Genomic Hybridization (Array CGH) has turned out a successful strategy to identify disease associated genes. Here we present the first systematic screen for chromosomal imbalances in ADHD using sub-megabase resolution Array CGH. To detect micro-deletions and -duplications which may play a role in the pathogenesis of ADHD, we carried out a genome-wide screen for copy number variations (CNVs) in a cohort of 99 children and adolescents with severe ADHD. Using high-resolution aCGH, a total of 17 potentially syndrome-associated CNVs were identified. The aberrations comprise four deletions and 13 duplications with approximate sizes ranging from 110 kb to 3 Mb. Two CNVs occurred de novo and nine were inherited from a parent with ADHD, whereas five are transmitted by an unaffected parent. Candidates include genes expressing acetylcholine-metabolising butyrylcholinesterase (BCHE), contained in a de novo chromosome 3q26.1 deletion, and a brain-specific pleckstrin homology domain-containing protein (PLEKHB1), with an established function in primary sensory neurons, in two siblings carrying a 11q13.4 duplication inherited from their affected mother. Other genes potentially influencing ADHD-related psychopathology and involved in aberrations inherited from affected parents are the genes for the mitochondrial NADH dehydrogenase 1 alpha subcomplex assembly factor 2 (NDUFAF2), the brain-specific phosphodiesterase 4D isoform 6 (PDE4D6), and the neuronal glucose transporter 3 (SLC2A3). The gene encoding neuropeptide Y (NPY) was included in a ~3 Mb duplication on chromosome 7p15.2-15.3, and investigation of additional family members showed a nominally significant association of this 7p15 duplication with increased NPY plasma concentrations (empirical FBAT, p = 0.023). Lower activation of the left ventral striatum and left posterior insula during anticipation of large rewards or losses elicited by fMRI links gene dose-dependent increases in NPY to reward and emotion processing in duplication carriers. Additionally, further candidate genes were examined via Matrix assisted laser desorption/ionization time of flight mass spectrometry (MALDI-TOF MS). This method enables the analysis of SNPs directly from human genomic DNA without the need for initial target amplification by PCR. All these findings implicate CNVs of behavior-related genes in the pathogenesis of ADHD and are consistent with the notion that both frequent and rare variants influence the development of this common multifactorial syndrome. The second part of this work concentrates on MLC1, a gene associated with Megalencephalic leukoencephalopathy with subcortical cysts, located on chromosome 22q13.33. To get more insight in the disease itself, a targeting vector for a conditional knockout mouse was constructed using homologous recombination. Furthermore, MLC1 has been suggested as a risk gene for schizophrenia, especially the periodic catatonia subtype. An initially identified missense mutation was found to be extremely rare in other patient cohorts; however, a recent report again argued for an association of two intronic MLC1 SNPs with schizophrenia and bipolar disorder. A case-control study of these polymorphisms as well as SNPs in the transcriptional control region of MLC1 was conducted in 212 chronic schizophrenic patients, 56 of which suffered from periodic catatonia, 106 bipolar patients, and 284 controls. Both intronic and promoter polymorphisms were specifically and significantly associated with periodic catatonia but not schizophrenia or bipolar disorder in general. A haplotype constructed from all polymorphisms was also associated with periodic catatonia. The MLC1 variation is associated with periodic catatonia; whether it constitutes a susceptibility or a modifier gene has to be determined.
Humans and other animals share choice preference for smaller-but-sooner over later-but-larger rewards, indicating that the subjective value of a reward is discounted as a function of time. This phenomenon referred to as delay discounting (DD), represents one facet of impulsivity which is inherently connected with reward processing and, within a certain range, adaptive. Maladaptive levels, however, can lead to suboptimal decision-making and represent important characteristics of psychopathologies such as attention-deficit/hyperactivity disorder (ADHD). In line with a proposed influence of dysregulated dopamine (DA) levels on impulsivity, neural structures involved in DD (the ventral-striatum [VS]; orbitofrontal cortex [OFC]) are highly innervated by dopaminergic neurons. However, studies explicitly testing the triadic interplay of dopaminergic neurotransmission, impulsivity and brain activation during intertemporal choice are missing. Therefore, the first study of the thesis examined the effect of different DA-bioavailability levels, indicated by a genetic polymorphism (Val158Met) in the gene of the catechol-O-methyltransferase, on the association of delay discounting and OFC activation. OFC response to monetary rewards that varied by delay-to-delivery was recorded with functional near-infrared spectroscopy (fNIRS) in a sample of 49 healthy human subjects. The results suggest a DA-related enhancement in OFC function from low (low DA level) to partial (intermediate DA level) and full (high DA level) reward delay sensitivity. Furthermore, DA-bioavailability was shown to moderate the association of neural reward delay sensitivity and impulsivity: OFC reward delay sensitivity was strongly correlated with impulsivity at intermediate DA-levels, but not at low or high DA-levels where impulsivity was related to delay-independent OFC amplitudes. It is concluded that DA-level should be considered as a crucial factor whenever impulsivity-related brain activation, in particular to reward delay, is examined in healthy subjects. Dysfunctional reward processing, accompanied by a limited ability to tolerate reward delays (delay aversion), has been proposed as an important feature in ADHD putatively caused by striatal hypo-dopaminergia. Therefore, the aim of the second study of this thesis was to examine subcortical processing of reward delays and to test for neural indicators of a negative emotional response to delay periods. Using functional magnetic resonance imaging (fMRI), brain activation in adult patients with ADHD (n=14) and healthy control subjects (n=12) was recorded during the processing of immediate and delayed rewards. Compared with healthy control subjects, hyporesponsiveness of the VS reward system was evident in patients with ADHD for both immediate and delayed rewards. In contrast, delayed rewards evoked hyperactivation in the dorsal caudate nucleus and the amygdala of ADHD patients, corroborating the central predictions of the delay aversion hypothesis. In combination both studies support the conception of a close link between delay discounting, brain activation and dopaminergic neurotransmission. The results implicate that studies on neural correlates of DD have to account for the DA-bioavailability level and for a negative emotional response to reward delays.
Mit der vorliegenden Arbeit soll die Ausprägung der Delinquenz und deren Vorhersagemöglichkeiten bei ehemaligen Patienten der Kinder- und Jugendpsychiatrie Würzburgs mit der Diagnose einer ADHS untersucht werden. Anhand von schrittweisen Diskriminanzanalysen wurden verschiedene Faktoren auf deren Vorhersagekraft für eine spätere Delinquenz untersucht. Dabei konnte mit sechs Variablen bei 74% der Probanden richtig zwíschen später vorhandener oder nicht vorhandener Delinquenz unterschieden werden. Noch bessere Ergebnisse wurden bei dem Versuch erzielt, Vergehen in bestimmten Deliktuntergurppen vorherzusagen.
Attention-deficit/hyperactivity disorder (ADHD) is a highly prevalent childhood-onset neurodevelopmental disorder that involves a substantial risk of persisting into adolescence and adulthood. A number of genome-wide screening studies in ADHD have been conducted in recent years, giving rise to the discovery of several variants at distinct chromosomal loci, thus emphasising the genetically complex and polygenic nature of this disorder. Accordingly, promising novel candidate genes have emerged, such as the gene encoding the glucose transporter isoform 3 (SLC2A3) and the gene encoding the latrophilin isoform 3 (LPHN3).
In this thesis, both genes were investigated in form of two separated projects. The first focused on SLC2A3 polymorphisms associated with ADHD and their potential physiological impact. For this purpose, gene expression analyses in peripheral cell models were performed as well as functional EEG measurements in humans. The second project concerned the murine gene Lphn3 including the goal of developing a mouse line containing a genetically modified Lphn3 with conditional knockout potential. In this respect, a specific DNA vector was applied to target the Lphn3 gene locus in murine embryonic stem (ES) cells as a prerequisite for the generation of appropriate chimeric mice.
The results of the first project showed that SLC2A3 duplication carriers displayed increased SLC2A3 mRNA expression in peripheral blood cells and significantly altered event-related potentials (ERPs) during tests of cognitive response control and working memory, possibly involving changes in prefrontal brain activity and memory processing. Interestingly, ADHD patients with the rs12842 T-allele, located within and tagging the SLC2A3 gene, also exhibited remarkable effects during these EEG measurements. However, such effects reflected a reversed pattern to the aforementioned SLC2A3 duplication carriers with ADHD, thus indicative of an opposed molecular mechanism. Besides, it emerged that the impact of the aforementioned SLC2A3 variants on different EEG parameters was generally much more pronounced in the group of ADHD patients than the healthy control group, implying a considerable interaction effect. Concerning the second project, preliminary results were gathered including the successful targeting of Lphn3 in murine ES cells as well as the production of highly chimeric, phenotypically unremarkable and
mostly fertile mouse chimeras. While germline transmission of the modified Lphn3 allele has not yet occurred, there are still several newborn chimeric mice that will be tested in the near future.
In conclusion, the findings suggest that SLC2A3 variants associated with ADHD are accompanied by transcriptional and functional changes in humans. Future research will help to elucidate the molecular network and neurobiological basis involved in these effects and apparently contributing to the complex clinical picture of ADHD. Moreover, given the increasing number of publications concerning latrophilins in recent years and the multitude of research opportunities provided by a conditional knockout of Lphn3 in mice, the establishment of a respective mouse line, which currently is in progress, constitutes a promising approach for the investigation of this gene and its role in ADHD.
Ziel der vorliegenden, experimentellen Arbeit war die Untersuchung der Persönlichkeitseigenschaft der Impulsivität bei adulten Patienten mit ADHS und vergleichbaren Versuchspersonen. Da die Ätiopathogenese erhöhter Impulsivität bei Patienten mit ADHS bislang ungeklärt ist, eine Beeinflussung des dopaminergen Systems durch vorausgehende Studien allerdings nahe liegt, wurden die verwendeten Impulsivitätsmessungen zusätzlich bezüglich einer möglichen Beeinflussung durch den Val158Met-COMT-Polymorphismus analysiert. Die Untersuchung beinhaltet mit 71 adulten Patienten mit ADHS und 54 gesunden Kontrollpersonen, die nach Alter, Geschlecht, IQ, Kopfumfang und Händigkeit vergleichbar waren, eine der aktuell größten Stichproben adulter Patienten mit ADHS. Während einer Stoppsignal-Aufgabe zur Erfassung der Antworthemmungsfähigkeit als Korrelat der Impulsivität wurden die Verhaltensdaten der Stichproben sowie die hirnphysiologischen Veränderungen mittels funktioneller Nahinfrarotspektroskopie aufgezeichnet und ausgewertet. Die erhobenen experimentellen Daten wurden anschließend mit selbstbewerteter Impulsivität des I7-Impulsivitätsfragebogens nach Eysenck verglichen und auf mögliche Zusammenhänge überprüft. Zudem wurden die beobachteten Ergebnisse der vorliegenden Arbeit auf einen möglichen Einfluss durch den Val158Met-COMT-Polymorphismus untersucht. Auf der Verhaltensebene zeigten Patienten mit ADHS im Vergleich zu gesunden Kontrollpersonen sowohl für die SSRZ als auch für die Go-RZ signifikant langsamere Reaktionszeiten. Im Vergleich der funktionellen Daten konnten für Patienten mit ADHS während der erfolgreichen Stopp-Trials signifikant bzw. tendenziell verminderte Aktivierungen im Bereich des IFC und DLPFC in beiden Hemisphären festgestellt werden. Die Untersuchung der selbstbewerteten Impulsivität anhand des I7-Fragebogens ergab ebenfalls einen signifikanten Unterschied zwischen den beiden Stichproben. Bezüglich der Zusammenhänge zwischen den einzelnen Impulsivitätsmessungen konnten für Patienten mit ADHS signifikant bzw. tendenziell negative Zusammenhänge zwischen SSRZ und Oxygenierung im rechten IFC sowie zwischen SSRZ bzw. Go-RZ und I7-Impulsivitätswerten festgestellt werden. In der Untersuchung des Einflusses durch den Val158Met-COMT-Polymorphismus ergab sich ein sehr heterogenes Bild, in dem sich keine eindeutig systematischen Genotyp- oder Interaktionseffekte zeigten. Während die beobachteten Befunde auf Verhaltensebene auf eine generelle Verlangsamung adulter Patienten mit ADHS im Vergleich zu gesunden Kontrollpersonen hindeuten, könnte das verminderte Aktivierungsmuster im Bereich des IFC und DLPFC während der erfolgreichen Stopp-Trials bei Patienten mit ADHS möglicherweise das zugrundeliegende funktionelle Korrelat zu den beobachteten Reaktionsunterschieden zwischen den beiden Stichproben darstellen. Obwohl Patienten mit ADHS im Vergleich zu gesunden Kontrollen aufgrund ihrer erhöhten Impulsivität defizitär erscheinen, deuten die Befunde bezüglich des Zusammenhangs zwischen selbstbewerteter und experimentell erhobener Impulsivität innerhalb der Patientengruppe einen möglichen Vorteil erhöhter I7-Impulsivitätswerte für die Reaktionsfähigkeit in der Stoppsignal-Aufgabe an. Bezüglich der Beeinflussung der erhobenen Daten durch den Val158Met-COMT-Polymorphismus lassen sich anhand der Befunde dieser Arbeit keine klaren Schlussfolgerungen ziehen.
EEG-Studie mit 34 erwachsenen ADHS-Patienten und 34 Kontrollpersonen mit einem modifizierten Eriksen-Flanker-Test. Dabei zeigten sich bei den ADHS Patienten verglichen mit der Kontrollgruppe verlängerte Reaktionszeiten, aber eine normale Fehleranzahl und ein normales post error slowing. Die ERN und die Pe waren vermindert. Dies weist auf Defizite in der Handlungsüberwachung bei erwachsenen ADHS-Patienten hin.
