The coast of Aqaba and the Aqaba region (Jordan) were investigated on their hydrogeo-ecosystem. The results of the research were translated into digits to build a geo-spatial data base. The fillings of the graben aquifer receive indirect type of recharge through the side wadis which drain the highlands. Surface water balance was modeled for a period of 20 years of daily climate records using MODBIL program which attributes direct recharge to wet years only. The hydrodynamic fresh water/seawater interface in the coastal zones was investigated by applying vertical geoelectric surveys and models of several methods to confirm its coincidence with the aquifer’s flow amounts, where human impacts in terms of over-pumping allowed more encroachment of seawater into land, and unintended recharge which led to seaward interface migration. A groundwater balance and solute transport were approached by developing a flow model from the hydrogeological and hydrochemical data. The nature of soil cover and aquifer whose physical properties enhance human impacts indicated the vulnerability of groundwater to pollution. This certainly threatens the marine ecology which forms the sink where the in-excess flow ends. The constructed digital background was exported into GIS to sub-zone the study area in terms of the aquifer’s vulnerability to pollution risks using DRASTIC index. However, it was unable to meet all geo-spatial factors that proved to have significant impacts on the vulnerability. Consequently, a comprehensive index -SALUFT- was developed. This suggests the suitable land use units for each zone in the light of vulnerability grades aiming at protecting the available groundwater resources.
Prolongierte Ischämieperioden des Herzens führen zu struktureller Schädigung der Kardiomyozyten, d.h. einer Desorganisation und Zerstörung des kontraktilen und plasmalemmalen Zytoskeletts, welche sich final durch Verlust an Kontraktilität und Ruptur der Plasmamembran manifestieren. Stressproteine können an die Komponenten des Zytoskelettes binden und durch Konformationsschutz der ischämischen Schädigung entgegenwirken. In vorangegangenen Untersuchungen wurde gezeigt, dass es unter Ischämie zu einer Translokation des konstitutiv in hoher Konzentration vorkommenden kardialen Stressproteins aB-Crystallin vom Zytosol an die Myofibrillen kommt. Dabei führen bereits kurzdauernde Ischämieperioden zu einer kompletten Umverteilung von aB-Crystallin in die Z/I-Region des Sarkomers. Es war das Ziel dieser Arbeit, diese Bindung von aB-Crystallin an Strukturproteine im Z/I-Banden Bereich näher zu charakterisieren und aB-Crystallin ultrastrukturell zu lokalisieren. Durch Immunogoldmarkierung konnte aB-Crystallin ultrastrukturell im Herzen unter globaler Ischämie in einer Linie parallel zur Z-Scheibe etwa in der Mitte der halben I-Bande lokalisiert werden. Diese Zone entspricht dem Bereich, der als N-Linie bezeichnet wird. In der Frage der in vivo-Bindungspartner von aB-Crystallin waren daher in erster Linie Komponenten der I-Bande, d.h. Aktin und Titin in Betracht zu ziehen. Z-Scheiben Proteine wie a-Actinin treten dagegen in den Hintergrund. Um Anhaltspunkte über die Bindungsstärke des Stressproteins aB-Crystallin an kardiale Myofibrillen unter Ischämie zu erhalten, wurden ischämische Myofibrillen aus dem Rattenherz mit hochmolaren Salzlösungen und chaotropen Substanzen behandelt. Dabei konnte eine sehr hohe Bindungsaffinität von aB-Crystallin an die Myofibrillen festgestellt werden. Die myofibrilläre Bindung zeigte sich resistent gegenüber 1M KCl, 1M NaSCN und 2M Harnstoff, erst 2M NaSCN und 4M Harnstoff, die eine Zerstörung der myofibrillären Integrität bewirken, vermögen die aB-Crystallin-Bindung zu lösen. Aktin dagegen ließ sich bereits durch 0,5M NaSCN-Lösung von den Myofibrillen extrahieren, Bedingungen, unter denen aB-Crystallin noch fest an die Myofibrillen gebunden blieb. Aktin scheidet somit als in vivo-Bindungspartner von aB-Crystallin aus. Dieses Ergebnis immunhistochemischer Untersuchungen konnte auch mit biochemischer Methodik (Immunreplikanalyse) verifiziert werden. Titin zeigte sich wie aB-Crystallin resistent gegenüber den meisten der oben aufgeführten Salzlösungen. Eine deutliche Extraktion von Titin aus den Myofibrillen konnte erst durch Behandlung mit 2M NaSCN sowie 4M Harnstofflösung beobachtet werden, das Extraktionsverhalten entsprach somit dem von aB-Crystallin. Einen Hinweis auf eine Assoziation von aB-Crystallin mit Titin lieferte der Nachweis von aB-Crystallin in isolierten Titinfraktionen aus ischämischen Herzen. Der direkte Beweis für eine aB-Crystallin-Titin-Interaktion konnte im Rahmen dieser Arbeit jedoch nicht erbracht werden. Bindungsstudien, die zwischen ausgewählten nativen, in vitro translatierten Titindomänen und aus der Linse isoliertem aB-Crystallin durchgeführt wurden, waren negativ. Dies ist möglicherweise dadurch bedingt, dass aB-Crystallin erst unter Ischämie mit Titin interagiert und in vitro Kofaktoren benötigt werden. Durch eine Bindung an I-Banden Abschnitte des Titinmoleküls könnte aB-Crystallin eine kardioprotektive Funktion erfüllen, indem es unter Ischämie stabilisierend auf das Filamentsystem einwirkt.
Mittels Laserblitz-Photolyse wurden die Triplettlebenszeiten sowie die Löschraten der Triplettzustände verschiedener Acetophenonderivate durch dG, 8-oxodG, DNA, molekularen Sauerstoff und die Ketone selbst bestimmt. Für AP-OAc, AP und BP wurden Triplettlebensdauern von 7-9 µs gemessen, während die Triplettzustände von AP-OH und AP-OtBu aufgrund alpha Spaltung deutlich kurzlebiger waren (ca. 1 µs); die alpha Spaltung konnte EPR-spektroskopisch durch Spinabfangexperimente mit DMPO und TEMPO belegt werden. Im Fall von AP-OMe wurde weder dessen Triplettzustand noch die Bildung von Radikalen detektiert, was auf einer schnell ablaufenden Norrish-Typ-II-Spaltung beruht. Aufgrund dieses photochemischen Verhaltens wurden die Ketone (mit Ausnahme von AP-OMe) in zwei Gruppen klassifiziert, nämlich die „Gruppe A“-Ketone (keine Radikalbildung) und die „Gruppe B“-Ketone (Radikalbildner). Während die „Gruppe A“-Ketone gegenüber niedrigen Konzentrationen von DNA (62.5 µM) inaktiv waren, verursachten die bei der Bestrahlung der „Gruppe B“-Ketone generierten Peroxylradikale, neben wenigen direkt induzierten Strangbrüchen, hauptsächlich die Guaninoxidationsprodukte 8-oxoGua und guanidinfreisetzende Produkte (GRP). Erst wenn die DNA-Konzentration zehnfach erhöht wird (625 µM), tritt bei der Photolyse der „Gruppe A“-Ketone auch DNA-Oxidation durch einen Elektronentransfer von der Guaninbase auf das angeregte Keton ein. Ein analoger Konzentrationseffekt wurde auch in der dG-Oxidation beobachtet, bei niedrigen Substratkonzentrationen sind nur die radikalbildenden „Gruppe B“-Ketone aktiv. Die Tatsache, dass in der dG-Oxidation durch die „Gruppe A“-Ketone kein 8-oxodG detektiert wurde, wurde auf dessen effiziente Oxidation durch dG•+-Radikalkationen zurückgeführt. Die „Gruppe B“-Ketone sind in Abwesenheit von O2 gegenüber dG und DNA oxidativ inaktiv, da die in der alpha Spaltung generierten kohlenstoffzentrierten Radikale keine Peroxylradikale bilden können. Die „Gruppe A“-Ketone sind gegenüber DNA in Abwesenheit wie auch in Anwesenheit von Sauerstoff genauso reaktiv, da der Elektronentransfer von DNA zum Keton unabhängig von Sauerstoff ist. Um mechanistische Einblicke in die oxidative DNA-Schädigung zu erlangen, wurden photochemische Modellstudien mit dem Nukleosid dG sowie 8-oxodG durchgeführt, wobei zusätzlich Spiroiminodihydantoin gebildet wird. Bis vor kurzem wurde die Struktur dieses Oxidationsproduktes als 4-HO-8-oxodG angenommen, dass zuerst in der dG Oxidation mit Singulettsauerstoff (1O2) beobachtet wurde. Weder Spiroiminodihydantoin noch 4 HO-8-oxodG sind als authentische Verbindungen bekannt, so dass eine zweifelsfreie Strukturaufklärung die Bestimmung der Konnektivität der markierten Positionen erforderte. Diese Zuordnung erfolgte mittels eines SELINQUATE-NMR Spektrums, mit dem schlüssig die 4 HO-8-oxodG-Struktur ausgeschlossen wurde. Wie alle „Gruppe B“-Ketone sind auch alle „Gruppe A“-Ketone in Abwesenheit von O2 mit Ausnahme von AP-OAc gegenüber dG inert. Dies ist ein Beleg dafür, dass der Elektronentransferschritt von dG zum Keton in Abwesenheit von Sauerstoff (im Gegensatz zur DNA-Oxidation) reversibel ist und daher keine Oxidation möglich ist, wenn die Ketylradikale nicht durch O2 abgefangen werden. Das aus AP-OAc gebildete Ketylradikal besitzt als einziges einen effektiven unimolekularen Deaktivierungsweg, nämlich die Acetation-abspaltung, so dass die Reversibilität nicht mehr möglich ist.
In einer früheren Arbeit berichteten wir, daß der monoklonale, für die DNA-topoisomerase IIalpha spezifische Antikörper Ki-S1 nicht nur die Kerne menschlicher A431-Zellen, sondern auch extranucleäre Strukturen färbt (Meyer et. al., 1997, J. Cell. Biol. 136, 775). Diese Strukturen waren wahrscheinlich Zentrosomen. In dieser Arbeit bestätige ich die Kolokalisation von Ki-S1 mit dem zentrosomalen Marker g-Tubulin. Darüber hinaus zeige ich die Erkennung der Zentrosomen durch Peptid-Antikörper gegen den N- und den C-Terminus der humanen Topoisomerase IIalpha, und sogar GFP-Chimären der Topoisomerase IIalpha (jedoch nicht IIbeta) in transfizierten Zellen wurden an den Zentrosomen detektiert. Die Färbung von Western blots isolierter Zentrosomen mit Topoisomerase IIalpha Antikörpern zeigte eine Proteinbande von 170kDA. Zudem zeigten isolierte Zentrosomen eine DNA-Decatenierungs und Relaxierungsaktivität, die mit der enthaltenen Menge an Topoisomerase IIalpha Protein korrelierte und mit gereinigtem rekombinanten Enzym vergleichbar ist. Die Toposimoerase IIa Epitope konnten weder durch Extraktion mit Salz, noch durch Behandlung mit DNase oder RNase von den Zentrosomen entfernt werden, Methoden, durch die das gesamte Enzym aus dem Kern entfernt wurde. Zusammengefasst legen diese Beobachtungen nahe, dass aktive Topoisomerase IIalpha fest und DNA unabhängig an das Zentrosom gebunden ist. Da wir auch in ruhenden Lymphozyten, die im Kern keine Topoisomerase IIalpha haben, zentrosomale Topoisomerase IIalpha finden konnten, nehmen wir an, daß es sich bei der zentrosomalen Variante um eine Speicherform handelt.
Ziel: Darstellung der verschiedenen Ellenbogenfrakturen im Kindesalter mit Ausnahme der suprakondylären Humerusfraktur bezüglich ihrer Häufigkeit, Therapiemöglichkeiten und typischen Komplikationen. Weiterhin Beurteilung der verschiedenen Therapiestrategien der einzelnen Frakturtypen unter Berücksichtigung der Schwere des Primärtraumas und der vorhandenen Begleitver-letzungen. Material und Methoden: Erfassung aller 73 kindlichen Patienten, die von 1984-1993 an der Chirurgischen Universitätsklinik mit Ellenbogenfrakturen ausgenommen der suprakondylären Humerusfraktur behandelt worden sind anhand der Krankenunterlagen und Bewertung der Ergebnisse der Nachuntersuchung von 48 Patienten 3-14 Jahre nach dem Unfall anhand der Klassifikation nach MORGER, welche auf dem Ausmaß von Bewegungseinschränkungen nach der Neutral-Null-Methode und Achsabweichungen der Ellenbogenachse in Grad basiert. Ergebnis: Insgesamt fand sich 21 mal ein ideales, 22 mal ein gutes, 1 mal ein befriedigendes und 4 mal ein schlechtes Ergebnis bei der Nachuntersuchung. Die häufigste Fraktur war die des Condylus radialis, gefolgt von der Epicondylus ulnaris Fraktur und der Fraktur des proximalen Radiusendes. Die übrigen Frakturen kamen nur selten vor. Bei den Condylus radialis Frakturen stellten sich Kompressionsosteosythesetechniken im Hinblick auf zu vermeidende Wachstumsstörungen als vorteilhaft gegenüber reinen Spickdrahtosteosynthesen dar. Bei den übrigen Frakturen konnte kein Osteosyntheseverfahren als eindeutig geeigneter beurteilt werden. Wichtig erscheint, dass bei Condylus radialis Frakturen und Epicondylus ulnaris Frakturen beim Vorliegen einer Fragmentdislokation und bei Frakturen des proximalen Radiusendes ab einem bestimmten Dislokationsgrad operative Therapieverfahren zur Anwendung kommen sollten. Insgesamt korreliert das Ergebnis der Nachuntersuchungen in erster Linie in entscheidendem Ausmaß mit der Schwere der Primärverletzung. Die Folgen starker Traumatisierungen des Kapsel-Band-Apparates stellten sich hierbei als besonders ungünstig und therapeutisch schwierig zu beeinflussend dar.
Diese Dissertation präsentiert Ergebnisse regionaler Niederschlagsabschätzungen für Namibia bei anthropogen verstärktem Treibhauseffekt, die mit der Methode des Statistischen Downscaling erzielt wurden. Über statistische Transferfunktionen werden Beziehungen zwischen großskaliger atmosphärischer Zirkulation und Namibischen Sommerregen aufgestellt. Dazu werden in einer 30-jährigen Kalibrierungsperiode Hauptkomponenten von Geopotentiellen Höhen verschiedener atmosphärischer Niveaus (300, 500, 1000hPa) mit den Niederschlagsmonatssummen (November bis März) von 84 Namibischen Stationen durch multiple Regressionsanalysen verknüpft, die für jede Station oder alternativ für Gitternetzniederschlagsdaten berechnet werden. Nach der Verifikation der statistischen Zusammenhänge in einem unabhängigen Zeitraum werden Regressionsmodelle jener Stationen bzw. Gitterpunkte selektiert, die mit signifikanten Korrelationen von r>0.4 zwischen beobachteten und modellierten Werten ausreichende Qualität garantieren. Diese Modelle werden eingesetzt, um unter Verwendung simulierter ECHAM3-T42 und ECHAM4tr-T42 Geopotentialdaten den lokalen Niederschlag für die jeweiligen Treibhauseffekt-Szenarien abzuschätzen. Als zusätzliche Methode, um die großskalige atmosphärische Zirkulation mit lokalen Stationsdaten zu verknüpfen, werden kanonische Korrelationsanalysen durchgeführt. Unabhängig von der Verfahrensweise resultieren für Klimabedingungen dreifacher bzw. transient ansteigender CO2-Konzentrationen im Vergleich zu einem Referenzzeitraum (1961-90) zunehmende Niederschläge in den nördlichen und östlichen Teilen Namibias von Dezember bis Februar. In den südlichen und südwestlichen Regionen sind von November bis Januar geringe Abnahmen zu verzeichnen. Die Abschätzungen für März zeigen einen deutlichen Rückgang der Niederschläge in ganz Namibia. Diese Ergebnisse weisen auf eine intensivierte, akzentuiertere Regenzeit hin, auch wenn die Gesamtmenge der Niederschläge unter Bedingungen des anthropogen verstärkten Treibhauseffekts mehr oder weniger gleich bleibt. Daher ist es von besonderer Bedeutung, die Abschätzungen der Niederschlagsänderungen auf monatlicher Ebene durchzuführen. Weitere Untersuchungen beinhalten die Trennung thermischer und dynamischer Effekte in den zur Abschätzung herangezogenen ECHAM3 und ECHAM4 Zirkulationsdaten. Durch die globale Erwärmung kommt es zu einer Anhebung der Geopotentiellen Höhen der Treibhauseffekt-Szenarien. Durch die Korrektur des Uplifting-Prozesses werden dynamisch induzierte Auswirkungen auf das Niederschlagsgeschehen erfasst. Áus der Verwendung uplifting-korrigierter Geopotentialdaten als Prädiktoren in der Downscaling-Prozedur resultieren sowohl im positiven als auch negativen Bereich geringere Änderungsraten in den Abschätzungsergebnissen. Ohne Zweifel reagiert das Klimasystem auf den anthropogen verstärkten Treibhauseffekt. In Bezug auf zukünftige Namibische Sommerregen ist es von besonderer Bedeutung die Auswirkungen des Treibhauseffekts regional und temporal zu differenzieren.
The proventriculus regulates the food passage from crop to midgut. As the haemolymph provides a constantly updated indication of an insect’s nutritional state, it is assumed that the factor controlling the proventri-culus activity is to be found in the haemolymph. The purpose of this doctoral thesis was to investigate how output (metabolic rate), input (food quality and food quantity) and internal state variables (haemolymph osmolarity and haemolymph sugar titer) affect each other and which of these factors controls the activity of the proventriculus in the honeybee. Therefore free-flying foragers were trained to collect con-trolled amounts of different sugar solutions. Immediately after feeding, metabolic rates were measured over different periods of time, then crop-emptying rates and haemolymph sugar titers were measured for the same individual bees. Under all investigated conditions, both the sugar transport rates through the proventriculus and the haemolyph sugar titers depended mainly on the metabolism. For bees collecting controlled amounts of 15 per cent, 30 per cent or 50 per cent sucrose solution haemolymph trehalose, glucose and fructose titers were constant for metabolic rates from 0 to 4.5 mlCO2/h. At higher metabolic rates, trehalose concentration decreased while that of glucose and fructose increased with the exception of bees fed 15 per cent sucrose solution. As the supply of sugar from the crop via the proventriculus was sufficient to support even the highest metabolic rates, the observed pattern must result from an upper limit in the capacity of the fat body to synthesise trehalose. The maximal rate of conversion of glucose to trehalose in the fat body was therefore calculated to average 92.4 µg glucose/min. However, for bees fed 15 per cent sucrose solution both the rate of conversion of glucose to trehalose and the rate of sugar transport from the crop to the midgut were limited, causing an overall decrease in total haemolymph sugar titers for metabolic rates higher than 5 mlCO2/h. Haemolymph sucrose titers were generally low but increased with increasing metabolic rates, even though sucrose was not always detected in bees with high metabolic rates. Though foragers were able to adjust their sugar transport rates precisely to their metabolic rates, a fixed surplus of sugars was transported through the proventriculus under specific feed-ing conditions. This fixed amount of sugars increased with increasing concentration and in-creasing quantity of fed sugar solution, but decreased with progressing time after feeding. This fixed amount of sugars was independent of the metabolic rates of the bees and of the molarity and viscosity of the fed sugar solution. As long as the bees did not exhaust their crop content, the haemolymph sugar titers were unaffected by the sugar surplus, by the time after feeding, by the concentration and by the viscosity of fed sugar solution. When bees were fed pure glucose (or fructose) solutions, un-usually little fructose (or glucose) was found in the haemolymph, leading to lower total haemolymph sugar titers, while the trehalose titer remained unaffected. In order to investigate the mechanisms underlying the regulation of the honeybee proven-triculus, foraging bees were injected either with metabolisable (glucose, fructose, trehalose), or non-metabolisable sugars (sorbose). Bees reacted to injections of metabolisable sugars with reduced crop-emptying rates, but injection of non-metabolisable sugars had no influence on crop emptying. Therefore it is concluded that the proventriculus regulation is controlled by the concentration of metabolisable compounds in the haemolymph, and not by the haemo-lymph osmolarity. A period of 10min was enough to observe reduced crop emptying rates after injections. It is suggested that glucose and fructose have an effect on the proventriculus activity only via their transformation to trehalose. However, when the bees were already in-jected 5min after feeding, no response was detectable. In addition it was investigated whether the overregulation is the result of feed-forward regulation for the imminent take-off and flight. In a first experiment, we investigated whether the bees release an extra amount of sugar solution very shortly before leaving for the hive. In a second experiment, it was tested whether the distance covered by the bees might have an influence on the surplus amount released prior to the take-off. In a third experiment, it was investigated if walking bees fail to release this extra amount of sugars, as they do not have to fly. Though we were not able to demonstrate that the overregulation is the result of feed-forward regulation for the imminent take-off and flight, it is conceivable that this phenome-non is a fixed reaction in foragers that can not be modulated. To investigate whether regulated haemolymph sugar titers are also observed in honeybee foragers returning from natural food sources, their crop contents and haemolymph sugar titers were investigated. While the quantity of the collected nectar was without influence on the haemolymph sugar titers, foragers showed increasing haemolymph sugar titers of glucose, fructose and sucrose with increasing sugar concentration of the carried nectar. In contrast no relationship between crop nectar concentrations and haemolymph trehalose titers was observed. We are sure that the regulation of food passage from crop to midgut is controlled by the trehalose titer. However, under some conditions the balance between consumption and income is not numerically exact. This imprecision depends on the factors which have an impact on the foraging energetics of the bees but are independent of those without influence on the foraging energetics. Therefore we would assume that the proventriculus activity is modulated by the motivational state of the bees.
There are ample sand dune and sand sheets in the Texas Rolling Plains, U.S.A. Their varied location, morphology and paleosol content pointed to differnces in their historical develpment throughout the Holocene. Younger dunes, so called fence line dunes have been identified as remnants of unsound agricultural practices which just recently formed at the beginning of this century. Correspondingly soils were eroded, in parts, down to the C-horizon in some of these areas. More mature sand were dated with the radiocarbon method and identified having formed during the Altithermal warming period. This study identifies major eolian anthropogenic and climatic reactivation and stabilisation phases in the Rolling Plains of Texas during the Holocene, but also ties them into the existing Southern High Plains and Great Plains climatic record. This study also researched the reasons for the regional and local sand reactivation phases and contributes to the eolian history in the Great Plains region. The outline of this dissertation is oriented towards a comprehensive regional approach in cultural and physical geography. Chapter 1 covers the physiographic setting of the Rolling Plains region including geology, geomorphology, climate and vegetation. Here the prerequisites for eolian activity in the area are explained, followed by the criteria for the selection of the individual study sites. In chapter 2 selected dune fields and sand sheets are introduced. Chapter 3 outlines the methodology as a combination of field research, laboratory analysis and remote sensing techniques, along with a brief interpretation of their application and success rate. Chapter 4 investigates interactive processes between the cultural development and the physical landscape of the region. The next 4 chapters are focusing on research results and interpretation. Chapter 5 interprets the youngest eolian episodes resulting from the cultural de-velopment of the area, including a description and definition of so called "fenceline dunes" and "shinnery motts". Other dunes with very young buried horizons are also described in this chapter, and a comparison with outcrops in the Nebraska Sand Hills is performed. Chapter 6 interprets short-term, cyclic, drought related sand reactivations several hundred years ago by means of a Post Oak (Quercus stellata) tree ring record as established by STAHLE and CLEAVELAND (1988). In chapter 7 older Holocene reactivation cycles are introduced, investigating the idea of the existence of a warmer period, previously named the Altithermal, which so far has only been identified in the Southern High Plains. The last chapter (8) includes a brief statement of the study’s purpose along with the summary and discussion of results presented. This chapter will end with further implications of this research.
The Hassberge escarpment landscape developed from a widely extended original etchplain surface which represents the Middle to Upper Miocene final stage of undifferentiated etchplain lowering within the study area. The decisive processes of Upper Miocene and Pliocene landform differentiation were continuing planationsurface lowering increasingly governed by the structural differentiation of the bedrock substrate and local expansion of planation surfaces. This formation led to valley cutting since the Pliocene/Pleistocene transition period in the course of progressive reduction of the areas still subject to planation. The extent of structural adaptation and the interpretation of correlative sediments confirm a differentiated course of Late Tertiary climatic change, which, within the overall trend towards lower temperatures since the Lower to Middle Miocene, expressed itself in increasingly dry conditions during the Late Upper Miocene and Pliocene (BOLDT 1997).
Taking the Lower Franconian scarplands (Main River region of southern Germany) as an example, the compatibility of the formation of stepped planation surfaces with the structural forcing of landform development is explained and confirmed in a world-wide comparison for regions of sedimentary rocks of varying resistance. The differentiated morphological context based on climate and geology is pointed out (BOLDT 1998).
Die zufällige Entdeckung von Tumoren der Nebenniere durch heute häufig eingesetzte bildgebende Verfahren führt zu einer Diagnose-Prävalenz solcher sogenannter Inzidentalome von 0,6-1,3 Prozent. Die mögliche subklinische Hormonaktivität im Sinne eines subklinischen Cushing-Syndroms und der in Verlaufsstudien festgestellte Progress – bis hin zum manifesten Cushing-Syndroms – erfordern ein sensitives Screening auf hormonelle Aktivität und engmaschige Verlaufskontrollen. In elf publizierten Fällen ist eine nahrungsabhängige Sonderform des Hyperkortisolismus vorgestellt worden. Ektope Rezeptoren für das Glukoseabhängige- Insulinotrope-Peptid in der Nebennierenrinde führten zu einem durch Glukoseabhängiges-Insulinotropes-Peptid vermittelten, nahrungsabhängigen Cushing-Syndrom. In einigen anderen Fällen wurden an Hypophysentumoren operierter Patienten mit zentralem CS Rezeptoren für gastrointestinale Botenstoffe nachgewiesen. Präoperativ war hier ebenfalls ein nahrungsabhängiges Kortisolprofil messbar. Subklinische Vorstufen dieser Pathophysiologien sind bisher nur in einem Fall einer durch Glukoseabhängiges-Insulinotropes-Peptid vermittelten Kortisolsekretion beschrieben. In unserer Studie untersuchten wir Patienten mit Inzidentalom auf das Vorliegen einer subklinischen Nahrungsabhängigkeit ihrer Kortisolsekretion. Vier der 15 Patienten zeigten einen durch orale Glukosebelastung hervorgerufenen Kortisolanstieg mit signifikant höheren Kortisolwerten als unter i.v.-Glukosebelastung oder i.v.-Infusion von Glukoseabhängigem-Insulinotropen-Peptid. Bei den Kontrollkollektiven aus gesunden Normalpersonen und Patienten mit Metabolischem Syndrom konnte dieses Phänomen nicht beobachtet werden. Der Kortisolanstieg erwies sich als ACTH-vermittelt. Es fanden sich bei diesen Patienten signifikant höhere Kortisolwerte im 24h-Sammelurin und niedrigere Nüchtern-ACTH-Werte im Vergleich zu den restlichen Inzidentalom-Patienten. Bei den von uns untersuchten Inzidentalom-Patienten ergaben sich keine Hinweise auf eine durch Glukoseabhängiges-Insulinotropes-Peptid vermittelte Kortisolsekretion, wohl aber auf das Vorliegen von die ACTH-Sekretion beeinflussenden, nahrungsabhängigen Faktoren, die noch zu identifizieren sind.
