Ziel der vorliegenden Studie war es, die Veränderungen der komplexen dreidimensionalen Infarktanatomie im Verlauf der Infarktheilung zu untersuchen. Material und Methoden: Mit Hilfe kernspintomographischer Late Enhancement (LE) Untersuchungen ist es möglich, den Myokardinfarkt im Verlauf der gesamten Infarktheilung abzubilden und exakt zu vermessen. Insgesamt wurden 74 LE Untersuchungen bei 30 Patienten nach erstmals aufgetretenem Myokardinfarkt durchgeführt. Alle Patienten waren einer Reperfusionstherapie unterzogen worden. Die Untersuchungszeitpunkte waren Tag 5±2 nach Myokardinfarkt (Mittelwert ± Standardabweichung, Zeitpunkt A), Tag 12±3 nach Myokardinfarkt (Zeitpunkt B), und nach 3 Monaten (Zeitpunkt C). 14 Patienten wurden zu allen drei Zeitpunkten untersucht, bei 10 Patienten wurden Messungen zu den Zeitpunkten A und C durchgeführt und bei 6 Patienten Messungen zu den Zeitpunkten B und C. In den LE Untersuchungen wurde der linke Ventrikel jeweils mit Hilfe von doppelt angulierten Kurzachsenschnitten (Schichtdicke 8 mm, ohne Zwischenschichtabstand) vollständig abgebildet. Die Bildauswertung erfolgte geblindet. In jedem Kurzachsenschnitt eines Infarkts wurden folgende Parameter gemessen: Die Infarktquerschnittsfläche (MI, in mm2), die größte zirkumferentielle Ausdehnung des Myokardinfarkts (Z, in mm) und die Infarktdicke (D, in mm). Aus diesen Parametern und der gegebenen Schichtdicke von 8 mm konnten das Infarktvolumen (IV, in mm3), die Infarktausdehnung (IA, in mm2), die mittlere Infarktdicke (MID, in mm) und die mittlere zirkumferentielle Ausdehnung des Infarkts (MIZ, in mm) berechnet werden. Die Infarktausdehnung ist hierbei eine Fläche und beschreibt den Infarkt in seiner longitudinalen und zirkumferentiellen Ausdehnung. Eine Zunahme der Infarktausdehnung ist gleichbedeutend mit einer Dilatation bzw. Expansion des Infarkts. Alle Parameter können unabhängig vom vitalen Restmyokard bestimmt werden. In einer zusätzlichen zweidimensionalen Analyse wurden außerdem die Veränderungen der Infarktanatomie am Ort der maximalen Infarktdicke zum Zeitpunkt der ersten Messung und die Veränderungen der Infarktanatomie am Ort der maximalen zirkumferentiellen Ausdehnung zum Zeitpunkt der ersten Messung untersucht. Die Ergebnisse werden als Mittelwerte mit dazugehörigen Standardfehlern berichtet. Zur Verlaufsbeurteilung wurden die Parameter der Zeitpunkte B und C als Prozentsatz der vorangegangenen Messungen berechnet. Ergebnis: Innerhalb der ersten drei Monate nach Infarkt wurde eine Verringerung des Infarktvolumens im Mittel auf 69±5% des Ausgangswertes beobachtet. Die Volumenabnahme war hierbei zu einem größeren Anteil auf die Abnahme der Infarktdicke und zu einem kleineren Anteil auf die Abnahme der Infarktausdehnung zurückzuführen. Die mittlere Infarktdicke nahm innerhalb von 3 Monaten im Mittel auf 79±3% und die Infarktausdehnung im Mittel auf 88±4% ab. Diese Veränderungen waren signifikant (p<0,05). Die Infarktausdehnung veränderte sich jedoch innerhalb des untersuchten Kollektivs unterschiedlich. Bei 75% der untersuchten Infarkte wurde die Infarktausdehnung im Verlauf der Untersuchung geringer, bei 25% nahm die Infarktausdehnung jedoch zu. Eine Abnahme der Infarktdicke wurde in 92% der Fälle beobachtet. Sie kam unabhängig von der Dilatation eines Infarkts vor. Die Infarktdicke verringerte sich zwischen dem Zeitpunkt A und C bei 5 von 6 Patienten mit einer Zunahme der Infarktausdehnung, aber auch bei 17 von 18 Patienten ohne Infarktdilatation. In der zweidimensionalen Analyse zeigte sich, dass die Veränderungen der Infarktdicke und der zirkumferentiellen Ausdehnung innerhalb eines Infarkts regional unterschiedlich stark ausgeprägt waren.
Die vorliegende prospektive Studie hatte zum Ziel, den Zusammenhang zwischen Depression und Mortalität einerseits und Lebensqualität und Mortalität andererseits bei chronischer Herzinsuffizienz zu untersuchen. Zusätzlich wurden Determinanten für Depression und Lebensqualität untersucht. Eine konsekutive Kohorte von 231 ambulanten Patienten mit chronischer Herzinsuffizienz wurde bei Studieneinschluss eingehend medizinisch untersucht und gebeten Fragebögen bezüglich Lebensqualität (KCCQ und SF-36) und Depression (PHQ) zu beantworten. Die Überlebensdaten wurden 2 bis 4 Jahre nach Studieneinschluss erhoben. In der vorliegenden Studie konnte ein Zusammenhang zwischen dem Vorliegen einer Major Depression und einer kürzeren Überlebenszeit nachgewiesen werden, der auch nach Kontrolle biomedizinischer prognostischer Faktoren bestand. Eine Minor Depression ging nicht mit einer kürzeren Überlebenszeit einher. Ferner kamen wir zu dem Ergebnis, dass der Schweregrad der NYHA-Klasse eine starke Determinante der Depression ist. Geschlecht, Alter und Ejektionsfraktion konnten nicht als Determinanten der Depression identifiziert werden. Auch die subjektiv empfundene Lebensqualität des Patienten steht im Zusammenhang mit der Überlebenszeit. Je höher die Lebensqualität, desto geringer ist das Risiko für Mortalität. Als Prädiktoren der Lebensqualität erwiesen sich Geschlecht, Alter, NYHA-Klasse und Depression, nicht jedoch die Ejektionsfraktion. Einschränkungen der Studie bestehen aufgrund der kleinen Stichprobe sowie des selektiven Patientenguts. Mögliche Mechanismen, die den Zusammenhang zwischen Depression und Überlebenszeit erklären können, sind verminderte Compliance des depressiven Patienten sowie unter anderem eine Dysregulation immunologischer Abläufe. Zur kausalen Klärung des Zusammenhangs von Depression bzw. Lebensqualität und Mortalität bedarf es zukünftig vor allem randomisierter Interventionsstudien.
Die Herzinsuffizienz ist eine Erkrankung, deren Prävalenz und Inzidenz weltweit ansteigen. Ihr Verlauf ist progredient und gleicht dem maligner Erkrankungen. Herzinsuffizienz ist eine Erkrankung des höheren Lebensalters und trotzdem sind ältere Patienten in den meisten randomisierten kontrollierten Therapiestudien unterrepräsentiert. Da herzinsuffiziente Frauen meist höheren Altersgruppen angehören, ist auch ihr Anteil in solchen Studien oft gering. Steigende Mortalität und Hospitalisierungsraten machen die Versorgung herzinsuffizienter Patienten zu einem medizinischen wie gesundheitsökonomischen Problem und weisen auf die Notwendigkeit einer Verbesserung der Versorgung hin. Außerdem liefert es Hinweise, dass evidenz-basierte Therapiemaßnahmen nur unzureichend bzw. nicht effektiv genug umgesetzt werden. Da in Deutschland Daten zur Epidemiologie und zur Versorgungslage der Herzinsuffizienz weitgehend fehlen, soll die Arbeit des Interdisziplinären Netzwerks Herzinsuffizienz (INH) Würzburg dahingehend einen Beitrag leisten. Vom 01.06.2002 bis zum 31.12.2003 wurden im INH-Register 1054 Patienten konsekutiv erfasst. Es handelte sich dabei um Patienten, die in der Medizinischen Klinik und der Medizinischen Poliklinik der Universität Würzburg mit der Diagnose chronische oder akut dekompensierte chronische Herzinsuffizienz ambulant oder stationär, notfallmäßig oder elektiv behandelt wurden. Die Aufnahme in das INH-Register erfolgte auf zwei verschiedene Arten. 367 Patienten (Gruppe A, 34,8%) stellten sich ambulant in der Herzinsuffizienz-Sprechstunde vor. 687 Patienten (Gruppe B, 65,2%) lehnten die ambulante Vorstellung entweder ab oder waren krankheitsbedingt, psychisch/mental oder aus logistischen Gründen dazu nicht in der Lage. Das INH-Register schloss Patienten jeden Alters, jeden Schweregrades und jeder Ätiologie ein und erfasste sowohl Patienten mit systolischer als auch mit nicht-systolischer Herzinsuffizienz. Die INH-Patienten wurden klinisch und technisch-apparativ hinsichtlich klinischem Profil und aktuellem Versorgungsstand untersucht. Wichtige Parameter dabei waren Alters- und Geschlechtsverteilung, Schweregrad und Ätiologie der Herzinsuffizienz, Komorbidität und aktuelle Pharmakotherapie. Folgende Ergebnisse konnten ermittelt werden:  Die Herzinsuffizienzpatienten im INH-Register waren älter und häufiger weiblich als in den meisten großen Herzinsuffizienztherapiestudien. Komorbiditäten, hervorzuheben Diabetes mellitus, Anämie, Niereninsuffizienz, Inflammation und Vorhofflimmern, waren ebenfalls häufig.  Ein Drittel der Patienten hatte eine erhaltene Pumpfunktion und davon waren über 50% Frauen. Patienten mit eingeschränkter Pumpfunktion waren häufiger männlich und häufiger infolge der Koronaren Herzkrankheit herzinsuffizient.  Die Umsetzung der leitliniengemäßen Pharmakotherapie betrug im Mittel 70%. Es zeichnete sich eine Behandlungsstrategie zu Ungunsten älterer und höhergradig herzinsuffizienter Patienten ab. Ebenfalls ließ sich eine Abhängigkeit der Qualität der Therapie vom Geschlecht nachweisen.  Im INH-Register konnte bestätigt werden, dass sich herzinsuffiziente Registerpatienten von herzinsuffizienten Studienpatienten bezüglich Alter, Geschlecht und Pumpfunktion unterscheiden. Hinweise für eine den Studien ähnelnde Bias konnten auch im INH-Register gefunden werden. Durch die Datensammlung des INH-Registers wurde deutlich, dass die herzinsuffizienten Patienten in der Allgemeinbevölkerung sich von denen großer Herzinsuffizienztherapiestudien unterscheiden. Es wurde aufgezeigt, dass die Umsetzung der leitlinienkonformen Therapie sich zwar im Laufe der letzten zehn Jahre verbessert hat, aber immer noch nicht ausreichend ist. Das INH-Register konnte eine Basis für die Ausarbeitung von Versorgungsstandards und Konzepten zur effektiven Umsetzung der leitliniengerechten Therapie schaffen.
Die vorliegende Arbeit befasste sich daher mit dem Effekt von Acarbose auf die durch Ischämie und Reperfusion verursachten myokardialen Schäden nach Saccharosebelastung bei gesunden Mäusen in vivo. Aus den Ergebnissen dieser Arbeit lässt sich ableiten, dass eine wiederholte postprandiale Hyperglykämie bei nicht - diabetischen Tieren ausreicht, um die durch Ischämie und Reperfusion verursachten Myokardschäden signifikant zu erhöhen. Dies kann durch eine Behandlung mit dem a - Glucosidase - Hemmer Acarbose verhindert werden.
Inhibition of Nuclear Import of Calcineurin Prevents the Development of Myocardial Hypertrophy

The Calcineurin/NFAT signaling cascade is a crucial transducer of cellular function. It has recently been emerged that in addition to the transcription factor NFAT, the phosphatase Calcineurin is also translocated to the nucleus. Our traditional understanding of Calcineurin activation via sustained high Ca2+-levels was also advanced by recent findings from this working group (AG Ritter), which showed that Calcineurin is activated by proteolysis of the C-terminal autoinhibitory domain. This leads to the constitutive activation and nuclear translocation of Calcineurin. Therefore, Calcineurin is not only responsible for dephosphorylating of NFAT in the cytosol thus enabling its nuclear import, its presence in the nucleus is also significant in ensuring the full transcriptional activity of NFAT. Formation of complexes between transcription factors and DNA regulates the transcriptional process. Therefore, the time that transcription factors remain nuclear is a major determinant of transcriptional activity. The movement of proteins over ~40 kDa into and out of the nucleus is governed by the nuclear pore complex (NPC). Transcription factors and enzymes that regulate the activity of these proteins are shuttled across the nuclear envelope by proteins that recognize nuclear localization signals (NLS) and nuclear export signals (NES) within the amino acid sequence of these transcription factors. In this study, the precise mechanisms of Calcineurin nuclear import and export were identified. Additionally to the nuclear localization sequence (NLS) and the nuclear export sequence (NES) within the sequence of Calcineurin, the respective nuclear cargo proteins, responsible for nuclear import, Importinβ1, and for nuclear export, CRM1, were identified. Inhibition of the Calcineurin/importin interaction by a competitive peptide, called Import Blocking Peptide (IBP), which mimicked the Calcineurin NLS, prevented nuclear entry of Calcineurin. A non-inhibitory control peptide showed no effect. Using this approach, it was able to prevent the development of myocardial hypertrophy. In Angiotensin II stimulated cardiomyocytes, both the transcriptional and the translational level was suppressed. Additionally, cell size and expression of Brain natriuretic peptide (as molecular marker for hypertrophy) were significantly reduced compared untreated controls. IBP worked dose-dependent, but did not affect the Calcineurin phosphatase activity. In conclusion, Calcineurin is not only capable of dephosphorylating NFAT, thus enabling its nuclear import, its presence in the nucleus is also important for full NFAT transcriptional activity. Using IBP to prevent the nuclear import of Calcineurin is a completely new approach to prevent the development of myocardial hypertrophy.
Im Rahmen einer randomisierten, prospektiven Cross-over-Studie wurden während der Hämodialyse an chronisch dialysepflichtigen Patienten standardisiert Parameter der Dialyseeffektivität und Biokompatibilität einer neuen, mittels einem Polyelektrolytadditiv modifizierten Dialysemembran, PUREMA® H, im Vergleich zu einer Kontrolldialysemembran, Helixone®, gemessen. Im Vergleich zur Kontrollmembran wies die PUREMA® H-Membran eine verbesserte Entfernung kleinmolekulargewichtiger Eiweiße incl. 2-Mikroglobulin und eine insbesondere für die Komplementaktivierung optimierte Biokompatibilität auf.
Im Mittelpunkt der Arbeit stand die Rolle der endothelialen Stickstoff-Monoxid-Synthase (eNOS) für die Endothelaktivierung. Für diese Untersuchungen wurde die MLEC-Zellkulturtechnik (murine lung endothelial cells) und die Gegenüberstellung des Wiltyp- und eNOS-Knockout-Genotyps verwendet. Die MLEC-Kulturen wurden aus dem mikrovaskulären Stromgebiet der Lungen von C57Bl6-Wildtyp-Mäusen (WT) und von eNOS-Knockout-Mäusen (KO) angelegt und immunomagnetisch (Anti-CD102) zweifach selektioniert. Die Reinheit der Kulturen für Endothelzellen nach zwei Selektionen lag bei über 95%. WT-Endothelzellen produzieren eine basale Menge an Stickstoff-Monoxid (NO). Sie steigern ihre NO-Produktion nach Stimulation mit VEGF (vascular endothelium growth factor), mit dem Kalzium-Ionophor Ionomycin sowie unter Scherkraftexposition. Die eNOS-Proteinexpression erhöht sich dementsprechend nach 12 Stunden Scherkraftexposition. WT- und eNOS-KO-Endothelzellen unterscheiden sich unter basalen Bedingungen nicht in ihrer Oberflächenexpression der Adhäsionsmoleküle ICAM-1, E-Selektin, P-Selektin und VCAM-1. Nach Zytokin-Stimulation erhöhen beide Genotypen ihr Adhäsionsmolekülprofil in gleicher Weise. Sowohl WT- als auch eNOS-KO-Endothelzellen verfügen zudem über einen schnellen Mechanismus, der die Hochregulation der P-Selektin-Oberflächenexpression nach Stimulation mit Thrombin oder Menadion in gleicher Weise ermöglicht. Auf Stimulation mit Thrombin oder Menadion reagieren WT-Zellen mit einem signifikanten Anstieg der Produktion von freien Sauerstoff-Radikalen (ROS, rapid oxygen species). eNOS-KO-Zellen zeigen eine im Vergleich zum WT erhöhte basale ROS-Produktion. Diese lässt sich auch nach Stimulation nicht weiter steigern. Die experimentellen Ergebnisse zeigen, dass die MLEC-Zellkulturtechnik ein verlässliches Modell für Untersuchungen an Gefäßendothelzellen darstellt. eNOS-KO-Zellen exprimieren nicht automatisch mehr Adhäsionsmoleküle an der Zelloberfläche als WT-Zellen. Allerdings ist die basale Produktion von ROS in eNOS-KO-Zellen vermehrt. Folglich ist in diesem Modell eNOS nicht für die konstitutive Suppression der endothelialen Aktivierung verantwortlich. Der NO-Effekt kann nicht in einer direkten und kontinuierlichen Unterdrückung der endothelialen Oberflächenaktivierung liegen. Das Fehlen von NO führt vielmehr zu einer Verschiebung des Gleichgewichts zwischen dem Radikalfänger NO und O2- (Superoxid) zugunsten von O2-. Aufgrund dieses Ungleichgewichts ist die basale ROS-Produktion von eNOS-KO-Zellen vermutlich erhöht. Damit wird die Endothelzelle empfindlicher gegenüber zusätzlichem oxidativen Stress. Die eNOS-KO-Zellen können die höhere ROS-Belastung in den durchgeführten Untersuchungen kompensieren. Es ist aber denkbar, dass bei zusätzlichem oxidativen Stress ein erhöhtes Maß an O2- das Startsignal für die Abläufe der endothelialen Aktivierung darstellt.
Kollagen Typ I, als wesentlicher Bestandteil der ECM, spielt eine entscheidende Rolle in der Wundheilung nach Myokardinfarkt. Zum einen ist eine ausreichende Narbenbildung zur Gewährleistung der Ventrikelstabilität notwendig, zum anderen führt eine überschießende Kollagensynthese mit interstitieller Fibrose des Myokards zu einer kontraktilen Dysfunktion des Ventrikels. Inwiefern sich eine Verminderung oder das Fehlen an Kollagen Typ I auf die Wundheilung und das Remodeling auswirkt, untersuchten wir am Modell der Osteogenesis Imperfecta Maus (OIM). 12-16 Wochen alte homozygote OIM Tiere, sowie heterozygote und homozygote Kontrollen, wurden einer Unterbindung der linken Koronararterie mit konsekutiven Myokardinfarkt (AMI) oder einer „Schein“- Infarzierung unterzogen. Echokardiographische Kontrollen der Ventrikelfunktion erfolgten am Tag vor, am Tag 1, Tag 8 und 8 Wochen nach AMI und „Schein“- Infarzierung, bevor wir die Tiere opferten. Das experimentelle Protokoll ex vivo zur Analyse der mechanischen Eigenschaften des Gewebes und des Kontraktionsverhaltens umfasste die Bestimmung der isometrischen Kraft und der Kraft- Frequenz- Beziehung. Außerdem wurden alle Herzen unabhängig vom Zeitpunkt des Todes histologisch aufgearbeitet 1. zur Infarktgrößenbestimmung, 2. zur immunhistologischen Bestimmung des Kollagengehalts und 3. zur Untersuchung der Todesursache bei vorzeitigem Tod. Vor Beginn der Studie fanden wir keine Unterschiede zwischen den OIM-/- und den Kontrollgruppen in ihrer Ventrikelfunktion. In der frühen Phase (Tag 3 bis 7) nach AMI war die Sterblichkeitsrate der OIM-/- aufgrund von Ventrikelrupturen signifikant erhöht verglichen mit den Kontrollen (54% OIM-/- vs. 13% WT). Wir konnten keine Abhängigkeit von der Infarktgrösse als ursächlichen Faktor auf das Entstehen einer Ruptur beobachten, da auch Tiere ohne makro- und mikroskopischen Nachweis eines Infarktes aus diesem Grund verstarben. Nach 8 Wochen präsentierten die OIM-/- eine signifikant niedrigere Dilatation des linken Ventrikels, sowie einen geringeren linksventrikulären Durchmesser verglichen mit den Kontrollgruppen. In den muskelphysiologischen Versuchen der isometrischen Kraftentwicklung konnte sowohl in der Infarkt- als auch in der Sham- Gruppe eine höhere maximale Kraft der OIM-/- verglichen mit den heterozygoten und homozygoten Kontrollen beobachtet werden. Zum Erreichen vergleichbarer Kraftniveaus war bei den homozygoten OIM eine signifikant grössere Vordehnung notwendig, was indirekt für eine höhere Gewebecompliance spricht. Der Kollagengehalt in der Infarktnarbe der OIM-/- war gegenüber den OIM+/- und WT Tieren signifkant erniedrigt. Keine Unterschiede in den drei Gruppen fanden sich in der Infarktgrössenentwicklung nach AMI.
Durch die Einführung eines neuen fallpauschalierten Abrechnungssystems (Diangnosis related groups, DRG) sowie zahlreiche neue Maßnahmen der Qualitätssicherung wächst der Anteil administrativer Tätigkeiten für die klinisch tätigen Ärztinnen und Ärzte. Diese Entwicklung hat nicht zuletzt auch Auswirkungen auf die zeitlichen Ressourcen, die für die ärztlichen Kernaufgaben zur Verfügung stehen. In Bereichen mit intensiver klinischer Forschung steigen darüberhinaus auch die Anforderungen an die forschungsbegleitende Dokumentation. Eine effiziente Unterstützung bei der Kalkulation, Planung und Durchführung von Therapien durch Informationstechnologische (IT-) Systeme kann eine sinnvolle Entlastung der Ärzte darstellen. Es wurde ein einsatzfähiger Prototyp entwickelt, prgrammiert und implementiert. Anschließend wurde das Ergebnis mit potentiellen Anwendern der Software evaluiert.
