Die Zeitschrift promptus – Würzburger Beiträge zur Romanistik erscheint einmal jährlich und wird durch den gemeinnützigen Verein promptus e.V. herausgegeben.
Sie richtet sich an alle Nachwuchswissenschaftler im Bereich der romanistischen Sprach- und Literaturwissenschaft sowie der Fachdidaktik und bietet diesen die Möglichkeit, in einem frühen Stadium ihrer akademischen Laufbahn qualitativ hochwertige Arbeiten zu publizieren. Zudem versteht sich die Zeitschrift als Impulsgeber für junge romanistische Forschung, ohne sich dabei thematisch zu beschränken.
Propagule pressure and an invasion syndrome determine invasion success in a plant community model

The success of species invasions depends on multiple factors, including propagule pressure, disturbance, productivity, and the traits of native and non-native species. While the importance of many of these determinants has already been investigated in relative isolation, they are rarely studied in combination. Here, we address this shortcoming by exploring the effect of the above-listed factors on the success of invasions using an individual-based mechanistic model. This approach enables us to explicitly control environmental factors (temperature as surrogate for productivity, disturbance, and propagule pressure) as well as to monitor whole-community trait distributions of environmental adaptation, mass, and dispersal abilities. We simulated introductions of plant individuals to an oceanic island to assess which factors and species traits contribute to invasion success. We found that the most influential factors were higher propagule pressure and a particular set of traits. This invasion trait syndrome was characterized by a relative similarity in functional traits of invasive to native species, while invasive species had on average higher environmental adaptation, higher body mass, and increased dispersal distances, that is, had greater competitive and dispersive abilities. Our results highlight the importance in management practice of reducing the import of alien species, especially those that display this trait syndrome and come from similar habitats as those being managed.
Beim idiopathischen Parkinson Syndrom (IPS) gewinnen nicht-motorische Symptome in Forschung und Klinik zunehmend an Bedeutung. So findet sich in der Literatur vermehrt Evidenz, dass die Propriozeption bei Patienten mit IPS (PmIPS) gestört ist. Verschiedene klinische und neuroanatomische Studien weisen darauf hin, dass es beim IPS zu einer fehlerhaften sensomotorischen Integration von propriozeptiven Informationen in den Basalganglien kommt. Zudem gibt es Hinweise, dass die passiv-sensible Wahrnehmung von Propriozeption pathologisch verändert ist. Außerdem wird vermutet, dass durch propriozeptives Training eine Verbesserung der Parkinsonsymptomatik erreicht werden kann. Ein spezielles Trainingsprogramm, die LSVT-BIG-Therapie, bei der gezielt trainiert wird, Bewegungen mit einer großen Amplitude durchzuführen, konnte motorische Symptome und Mobilität beim IPS effektiv verbessern.
In der vorliegenden Arbeit stellten wir folgende Hypothesen auf: Das IPS geht mit einer fehlerhaften sensomotorischen Integration von Propriozeption einher. Die afferente propriozeptive Wahrnehmung ist ebenfalls pathologisch verändert. Eine propriozeptive Rekalibrierung ist mithilfe der LSVT-BIG-Therapie möglich.
Für die Überprüfung dieser Hypothesen schlossen wir 30 PmIPS und 15 gesunde Probanden in unsere Fall-Kontroll-Studie ein und führten eine Eingangsuntersuchung durch. 11 PmIPS absolvierten anschließend eine vierwöchige LSVT-BIG-Therapie. Die Folgeuntersuchungen fanden 4 und 8 Wochen nach der Eingangsuntersuchung statt.

Diese beinhalteten neuropsychologische Testungen, außerdem die Bestimmung der Lebensqualität, die Erhebung des motorischen Teils der Movement Disorder Society Unified Parkinson´s Disease Rating Scale (MDS-UPDRS III), Untersuchungen zur Feinmotorik, die Durchführung einer diagnostischen Transkraniellen Magnetstimulation (TMS) sowie Testverfahren zur Propriozeption, darunter sowohl Zeigeversuche, als auch die Bestimmung der Position einer Extremität, ohne visuelle Kontrolle.
Die Ergebnisse zeigten, dass die IPS-Gruppe gegenüber der gesunden Kontrollgruppe signifikant größere Zeigefehler machte, wohingegen die Bestimmung der Position einer Extremität in beiden Gruppen vergleichbar präzise möglich war. Zusätzlich zeigte eine von sieben Messungen der Feinmotorik einen signifikanten Unterschied zwischen PmIPS und Kontrollen. Die Messungen der TMS erbrachten hingegen keine signifikant messbaren Unterschiede zwischen den Gruppen.
In den Folgeuntersuchungen nach therapeutischer Intervention ergaben die Zeigeübungen eine signifikante Verbesserung der BIG-Gruppe im Zeitverlauf. Die Untersuchungen zu Feinmotorik und MDS-UPDRS III ergaben zwar eine tendenzielle Verbesserung durch die LSVT-BIG-Therapie, waren jedoch statistisch nicht signifikant.
Die Lebensqualität der PmIPS in der BIG-Gruppe verbesserte sich signifikant nach Intervention.
Die Ergebnisse sprechen für die Hypothese der fehlerhaften propriozeptiven Integration beim Morbus Parkinson. Dies zeigte sich für aktive sensomotorische Tasks, nicht hingegen in der passiv-sensiblen propriozeptiven Testung.
Auch wenn weitere Studien mit größeren Kohorten benötigt werden, legt unsere Studie nahe, dass die LSVT-BIG-Therapie mit einer propriozeptiven Rekalibrierung einhergeht. Damit erklärt sich möglicherweise der nachhaltige Erfolg der Therapie.
Aufgrund der divergierenden Studienlage bezüglich der physiotherapeutischen Nachbehandlung nach operativer Rotatorenmanschettenrefixation erfolgte im Rahmen einer prospektiv randomisierten Studie die Evaluation zweier Nachbehandlungsmodelle nach operativer Refixation vollschichtiger RM-Rupturen in Mini-Open-Technik. Hierfür wurden 57 Patienten präoperativ, 3 Wochen, 6 Wochen sowie 6 Monate postoperativ nachuntersucht und ausgewertet. Die Scores beinhalteten den NRS-Score, Constant-Score, DASH-Score, ASES-Score, NHP-Score, SF-36-Score sowie eine sonographische Untersuchung zur Beurteilung der Reruptur nach 6 Monaten postoperativ.
Einheitlich erfolgte die Ruhigstellung im Gilchrist-Verband für 6 Wochen. In der konservativen Nachbehandlungsgruppe wurden bis 6 Wochen postoperativ lediglich Pendelübungen durchgeführt, in der progressiven Nachbehandlungsgruppe erfolgte eine passive Beübung direkt postoperativ bis an die Schmerzgrenze mit Ausnahme der Adduktion.
Im Gesamtkollektiv war eine Rerupturrate von 5,3% zu verzeichnen mit 3,7% in der konservativen und 6,7% in der progressiven Nachbehandlungsgruppe ohne signifikanten Gruppenunterschied (p=0,540). Bezüglich der klinischen und psychischen Ergebnisse zeigte sich 6 Monate postoperativ lediglich eine Einschränkung der aktiven Außenrotation in der konservativen Nachbehandlungsgruppe (46,2∘ vs. 39,7∘, p=0,031), sonst war kein signifikanter Gruppenunterschied zu sehen.
Weiterhin erfolgten Subgruppenanalysen insbesondere hinsichtlich Alter und Geschlecht der Patienten. Dabei haben Patienten über 65 Jahren unabhängig von der Nachbehandlungsgruppe kürzer Analgetika eingenommen und waren 6 Wochen postoperativ weniger bewegungseingeschränkt. Aufgrund einer Tendenz zu vermehrten Rerupturen nach progressiver Nachbehandlung in der Literatur werden daher weiterführende Studien benötigt um zu evaluieren, ob ältere Patienten von einer vermehrten Ruhigstellung profitieren könnten.
Diese Studie präsentiert im Gegensatz zu der überwiegend in der Literatur verwendeten arthroskopischen OP-Technik Ergebnisse nach RM-Refixation in Mini-Open-Technik. Damit liefert sie eine gute Grundlage für weiterführende Studien insbesondere in der Behandlung von größeren RM-Rupturen, welche ein erhöhtes Rerupturrisiko besitzen und von einer konservativen Nachbehandlung profitieren könnten.
Die Invasive Aspergillose (IA) stellt eine Hauptursache der infektassoziierten Morbidität und Mortalität bei pädiatrischen Patienten mit hämato-onkologischer Grunderkrankung und/oder allogener Stammzelltransplantation dar. Die sichere und frühzeitige Diagnose ist bei Kindern aufgrund spärlicher pädiatrischer Daten weiterhin eine klinische Herausforderung. Die Kombination der Biomarker Galactomannanantigen und Aspergillus DNA hat sich in Erwachsenenstudien als vorteilhaft in der Diagnose der IA erwiesen. Ziel der durchgeführten Studie war daher, die diagnostische Güte des kombinierten Biomarkerscreenings in einer pädiatrischen Hochrisikokohorte zu ermitteln.
Hierfür wurden 39 pädiatrische Patienten, die während eines Zeitraumes von drei Jahren aufgrund einer hämato-onkologischen Grunderkrankung und Notwendigkeit einer Stammzelltransplantation in der Würzburger Kinderklinik behandelt wurden, einem hochstandardisierten, zweimal wöchentlichen Screening auf Galactomannanantigen und fungaler DNA zugeführt. Zusätzlich wurde für jeden Patienten ein breites Spektrum an klinischen Daten sowie mikrobiologischen und radiologischen Ergebnissen erfasst und die IA-Klassifikation nach den EORTC/MSG-Kriterien durchgeführt.
Unsere Daten zeigten eine IA-Inzidenz (probable IA) von 10%, was per definitionem einer Hochrisikokohorte entspricht. Das kombinierte Monitoring der Biomarker Galactomannanantigen und Aspergillus-DNA wies eine hohe diagnostische Genauigkeit mit einer Sensitivität/Spezifität/PPV/NPV von 1.00 und gute Eignung als Screeningtest auf. Die antifungale Prophylaxe zeigte keinen negativen Einfluss auf die diagnostischen Gütekriterien der beiden Biomarker, wie in anderen Studien postuliert. Der Galactomannanindex erwies sich als vielversprechender Surrogatmarker für das Outcome und das Therapieansprechen.
Weiterführende Studien sind notwendig, um festzulegen, ob die Biomarkerkombination eine Detektion asymptomatischer subklinischer Infektionen als eine Art „Frühwarnsystem“ ermöglicht und somit eine Reduktion der Mortalität bedingen kann.
Climate change is increasing the frequency and intensity of warming and drought periods around the globe, currently representing a threat to many plant species. Understanding the resistance and resilience of plants to climate change is, therefore, urgently needed. As date palm (Phoenix dactylifera) evolved adaptation mechanisms to a xeric environment and can tolerate large diurnal and seasonal temperature fluctuations, we studied the protein expression changes in leaves, volatile organic compound emissions, and photosynthesis in response to variable growth temperatures and soil water deprivation. Plants were grown under controlled environmental conditions of simulated Saudi Arabian summer and winter climates challenged with drought stress. We show that date palm is able to counteract the harsh conditions of the Arabian Peninsula by adjusting the abundances of proteins related to the photosynthetic machinery, abiotic stress and secondary metabolism. Under summer climate and water deprivation, these adjustments included efficient protein expression response mediated by heat shock proteins and the antioxidant system to counteract reactive oxygen species formation. Proteins related to secondary metabolism were downregulated, except for the P. dactylifera isoprene synthase (PdIspS), which was strongly upregulated in response to summer climate and drought. This study reports, for the first time, the identification and functional characterization of the gene encoding for PdIspS, allowing future analysis of isoprene functions in date palm under extreme environments. Overall, the current study shows that reprogramming of the leaf protein profiles confers the date palm heat- and drought tolerance. We conclude that the protein plasticity of date palm is an important mechanism of molecular adaptation to environmental fluctuations.
Protein Kinase D2 drives chylomicron‐mediated lipid transport in the intestine and promotes obesity

Lipids are the most energy‐dense components of the diet, and their overconsumption promotes obesity and diabetes. Dietary fat content has been linked to the lipid processing activity by the intestine and its overall capacity to absorb triglycerides (TG). However, the signaling cascades driving intestinal lipid absorption in response to elevated dietary fat are largely unknown. Here, we describe an unexpected role of the protein kinase D2 (PKD2) in lipid homeostasis. We demonstrate that PKD2 activity promotes chylomicron‐mediated TG transfer in enterocytes. PKD2 increases chylomicron size to enhance the TG secretion on the basolateral side of the mouse and human enterocytes, which is associated with decreased abundance of APOA4. PKD2 activation in intestine also correlates positively with circulating TG in obese human patients. Importantly, deletion, inactivation, or inhibition of PKD2 ameliorates high‐fat diet‐induced obesity and diabetes and improves gut microbiota profile in mice. Taken together, our findings suggest that PKD2 represents a key signaling node promoting dietary fat absorption and may serve as an attractive target for the treatment of obesity.
The liver plays a pivotal role in maintaining energy homeostasis. Hepatic carbohydrate and lipid metabolism are tightly regulated in order to adapt quickly to changes in nutrient availability. Postprandially, the liver lowers the blood glucose levels and stores nutrients in form of glycogen and triglycerides (TG). In contrast, upon fasting, the liver provides glucose, TG, and ketone bodies. However, obesity resulting from a discrepancy in food intake and energy expenditure leads to abnormal fat accumulation in the liver, which is associated with the development of hepatic insulin resistance, non-alcoholic fatty liver disease, and diabetes. In this context, hepatic insulin resistance is directly linked to the accumulation of diacylglycerol (DAG) in the liver. Besides being an intermediate product of TG synthesis, DAG serves as second messenger in response to G-protein coupled receptor signaling. Protein kinase D (PKD) family members are DAG effectors that integrate multiple metabolic inputs. However, the impact of PKD signaling on liver physiology has not been studied so far. In this thesis, PKD3 was identified as the predominantly expressed isoform in liver. Stimulation of primary hepatocytes with DAG as well as high-fat diet (HFD) feeding of mice led to an activation of PKD3, indicating its relevance during obesity. HFD-fed mice lacking PKD3 specifically in hepatocytes displayed significantly improved glucose tolerance and insulin sensitivity. However, at the same time, hepatic deletion of PKD3 in mice resulted in elevated liver weight as a consequence of increased hepatic lipid accumulation. Lack of PKD3 in hepatocytes promoted sterol regulatory element-binding protein (SREBP)-mediated de novo lipogenesis in vitro and in vivo, and thus increased hepatic triglyceride and cholesterol content. Furthermore, PKD3 suppressed the activation of SREBP by impairing the activity of the insulin effectors protein kinase B (AKT) and mechanistic target of rapamycin complexes (mTORC) 1 and 2. In contrast, liver-specific overexpression of constitutive active PKD3 promoted glucose intolerance and insulin resistance. Taken together, lack of PKD3 improves hepatic insulin sensitivity but promotes hepatic lipid accumulation. For this reason, manipulating PKD3 signaling might be a valid strategy to improve hepatic lipid content or insulin sensitivity. However, the exact molecular mechanism by which PKD3 regulates hepatocytes metabolism remains unclear.
Unbiased proteomic approaches were performed in order to identify PKD3 phosphorylation targets. In this process, numerous potential targets of PKD3 were detected, which are implicated in different aspects of cellular metabolism. Among other hits, phenylalanine hydroxylase (PAH) was identified as a target of PKD3 in hepatocytes. PAH is the enzyme that is responsible for the conversion of phenylalanine to tyrosine. In fact, manipulation of PKD3 activity using genetic tools confirmed that PKD3 promotes PAH-dependent conversion of phenylalanine to tyrosine. Therefore, the data in this thesis suggests that PKD3 coordinates lipid and amino acid metabolism in the liver and contributes to the development of hepatic dysfunction.
Protocadherine (Pcdh) sind im zentralen Nervensystem (ZNS) stark exprimiert und üben vielfältige Funktionen bei der neuronalen Entwicklung aus. Der Knockout eines Vertreters der Pcdhs, PcdhgC3, führt zu Veränderungen in tight junction Proteinleveln in mikrovaskulären Endothelzellen der Blut-Hirn-Schranke (BBB). In dieser Arbeit untersuche ich die Rolle des PcdhgC3 Knockouts (KO) in Transportern der Blut-Hirn-Schranke sowie dessen Auswirkungen auf die Signaltransduktion mittels Serumreduktion, Zellmigrationsversuchen, Signalweg-Inhibierung und Sauerstoff-Glucose-Entzug. Der PcdhgC3 Knockout resultiert in veränderten Proteinleveln der BBB Transporter und könnte ein vielversprechendes Therapieziel zukünftiger Pharmakotherapie sein. Ebenso führt die Serumreduktion in den KO-Zellen zu höheren Leveln von Signalkinasen (Erk). Die Knockout-Zelllinie zeigt signifikant schnellere Migrationsraten und scheint durch Signalweg-Inhibitoren (mTOR, MAPK, wnt-Inhibitoren) stärker im Wachstum reduziert zu sein. So könnte PcdhgC3 eine Rolle bei der Regulierung von Signalwegen spielen und zu einer veränderten Integrität der Blut-Hirn-Schranke beitragen.
Proximity dimensions and the emergence of collaboration: a HypTrails study on German AI research

Creation and exchange of knowledge depends on collaboration. Recent work has suggested that the emergence of collaboration frequently relies on geographic proximity. However, being co-located tends to be associated with other dimensions of proximity, such as social ties or a shared organizational environment. To account for such factors, multiple dimensions of proximity have been proposed, including cognitive, institutional, organizational, social and geographical proximity. Since they strongly interrelate, disentangling these dimensions and their respective impact on collaboration is challenging. To address this issue, we propose various methods for measuring different dimensions of proximity. We then present an approach to compare and rank them with respect to the extent to which they indicate co-publications and co-inventions. We adapt the HypTrails approach, which was originally developed to explain human navigation, to co-author and co-inventor graphs. We evaluate this approach on a subset of the German research community, specifically academic authors and inventors active in research on artificial intelligence (AI). We find that social proximity and cognitive proximity are more important for the emergence of collaboration than geographic proximity.
Prädiktion des Verschlusses großer intrakranieller Arterien anhand präklinischer Schlaganfallscores

2015 konnte in mehreren Studien ESCAPE, EXTENDED IA, MR CLEAN, REVASCAT, SWIFT-PRIME eine signifikante Überlegenheit der mechanischen Thrombektomie verglichen mit der alleinigen i. v. Lysetherapie mit rtPA bezogen auf Revaskularisierung bei Patienten mit einer LVO (large vessel occlusion) nachgewiesen werden. Diese neue Therapiemöglichkeit erforderte eine Aufteilung der Patienten die von einer Thrombektomie profitieren (LVO) und der Patienten, die keiner Thrombektomie zugeführt werden können (nLVO). Die zentrale Fragestellung der Studie ist: Kann ein symptomorientierter Schlaganfallscore die Wahrscheinlichkeit eines großen intrakraniellen Gefäßverschlusses mit hinreichender Präzision vorhersagen und kann auf Basis dieser Vorhersage ein Patient direkt in ein übergeordnetes Schlagfanfallzentrum gebracht werden, obwohl sich dadurch eine Bridging Lysetherapie verzögern würde?
Um diesen Fragen auf den Grund zu gehen führten wir eine monozentrische Querschnittstudie durch, in deren Rahmen 215 Patienten rekrutiert wurden. Die Rekrutierung erfolgte mittels eines aus Subitems bereits etablierter Schlafanfallscores (FAST, CPSS, LAPSS, 3ISS, RACE), zusammengesetzten Fragebogens. Die ausgefüllten Fragebögen wurde in Excel digitalisiert und mittels SPSS, Signifikanz und Odds Ratio berechnet. Anschließend wurde aus den signifikanten Subitems mit der höchsten Odds Ratio ein neuer einfach anzuwendender Schlaganfallscore, bestehend aus den präklinisch erhobenen Daten gebildet (Würzburg Score of Large Vessel Occlusions, WOLVE- Score). Weiter wurden Signifikanz, Odds Ratio, Sensitivität und Spezifität des WOLVE-Score mit denen der oben genannten etablieren Scores verglichen.
Hintergrund: Die transarterielle Chemoembolisation (TACE) stellt eine Erstlinientherapie bei nicht resezierbarem HCC im intermediären Stadium (BCLC B) dar. TACE induziert einen zytotoxischen und ischämischen Gewebeeffekt, der möglicherweise zu einer Leberfunktionsstörung führt. Der 13C-Methacetin-Atemtest (MBT) ist ein nichtinvasiver CYP1A2-Funktionstest zur Beurteilung der funktionellen Leberzellmasse. Ziel dieser prospektiven Studie war es, die Auswirkung der konventionellen TACE auf die hepatozelluläre Reserve, gemessen mittels 13C-MBT, statischen Leberfunktionstests und entzündlichen Parametern bewerten zu können.
Methoden & Ergebnisse: 27 Patient*innen mit nicht resezierbarem HCC (BCLC B, Child Pugh A) erhielten vor (d0), 24 Stunden (d1) und 72 Stunden (d3) nach 41 cTACE-Verfahren einen MBT. Das hepatische Lipiodol®-Verteilungsvolumen wurde aus CT-Daten berechnet. Statische Leberfunktionstests, entzündliche Parameter und klinische Ereignisse wurden an d0-3 analysiert. Es zeigte sich eine deutliche Verringerung der CYP1A2-Funktion nach cTACE an d1 und d3, was hauptsächlich durch die Entzündungsreaktion (CRP) und hepatozelluläre Schadensmarker (AST) und nur in geringem Maße durch das embolisierte Lebervolumen zu erklären ist.
Schlussfolgerung: Der MBT kann die kurzfristige Verringerung der Leberfunktionsreserve sensitiv abbilden und korreliert mit klinischen Komplikationen nach cTACE. Der MBT kann Anwendung in der frühen Identifizierung einer hepatischen Dysfunktion finden.
Eine chronische Infektion mit dem Hepatitis B Virus (HBV) ist ein wichtiges, jedoch vernachlässigtes Problem der globalen Gesundheit. Weltweit sind ungefähr 257 Millionen Personen chronisch mit dem Virus infiziert. Eine Impfung gegen diese Erkrankung ist seit 1982 verfügbar. Impfprogramme haben dort, wo die Impfung ausreichend verfügbar ist, einen durchschlagenden Erfolg in der Reduktion von chronischer Hepatitis B gezeigt. Es gibt jedoch Teile der Welt, in denen die Impfung nicht zu einem ausreichenden Schutz führt, da die Infektion schon vor der Gabe der ersten Impfdosis geschieht oder die Impfung nicht oder nur unvollständig verabreicht wird. In Ländern mit einer hohen Prävalenz von Hepatitis B wird das Virus vor allem von Müttern auf Kinder übertragen. Kinder, die sich innerhalb ihres ersten Lebensjahres infizieren, haben hohes Risiko eine chronische Infektion zu entwickeln. Tansania, mit einer Prävalenz der chronischen Hepatitis B von 7,2% in der Allgemeinbevölkerung, gilt als Hochendemiegebiet der Erkrankung.
Die Weltgesundheitsorganisation (WHO) nennt in ihrer globalen Strategie zur Elimination von viralen Hepatitiden die Prävention von Mutter-Kind-Übertragung als eine zentrale Aufgabe. Das beinhaltet unter anderem die Testung von schwangeren Frauen und die Impfung von Neugeborenen gegen HBV innerhalb von 24 Stunden nach der Geburt. Diese Maßnahmen sind jedoch in Tansania nicht implementiert. Zudem lagen zum Durchführungszeitraum der Studie nur wenige Daten zu der Prävalenz von HBV in schwangeren Frauen vor.
Aus diesem Grund untersuchte die Studie die Prävalenz von Hepatitis B unter schwangeren Frauen in einem Krankenhaus der tertiären Versorgungsstufe in Mwanza, Tansania, sowie den sozio-ökonomischen Hintergrund und mögliche Risikofaktoren für eine Infektion seitens der Mütter. Ergänzend wurden verschiedene serologische und virologische Analysen des Blutes der Mütter und ihrer Kinder durchgeführt.
Im Rahmen einer retrospektiven Datenanalyse über einen Zeitraum von 6 Jahren wurde an Hand von 814 operierten Hoden die Koinzidenz eines Maldescensus testis mit einer Morgagni-Hydatide untersucht. Dabei ergab sich eine Hydatidengesamtprävalenz bei erfolgter Hodenhülleneröffnung nicht-deszendierter Hoden von 62,66 %. Zusätzlich konnten wir eine Korrelation zwischen Patientenalter und Größenausprägung der Hydatiden feststellen. Entsprechend der Studienergebnisse kann die Vermutung geäußert werden, dass von einer Größenwachstumsdynamik der Hydatiden – am ehesten unter Hormoneinfluss – in Bezug zum Patientenalter ausgegangen werden kann.
1p36 deletion syndrome represents the most common terminal deletion observed in humans. Major clinical findings comprise developmental delay/intellectual disability, poor or absent expressive language, congenital central muscular hypotonia, brain anomalies, brachydactyly/camptodactyly, short feet, and characteristic facial features like straight eyebrows, deep-set eyes, and midface hypoplasia. So far, there is very limited knowledge about comorbid psychiatric disorders and their effective treatment in this special population. To fill this gap, this case report presents an initially four-year-old girl with 1p36.33–1p36.32 deletion, moderate intellectual disability, insomnia, oppositional-defiant disorder and attention deficit/hyperactivity disorder covering a period of time of about 1.5 years comprising initial psychological/psychiatric assessment, subsequent day clinic/outpatient treatment (amongst others including off-label use of melatonin and methylphenidate as well as parent-child interaction therapy) and follow-up assessment. Follow-up results indicated good efficacy of melatonin and methylphenidate medication without any adverse effects. Multidisciplinarity in diagnosis and treatment are mandatory to meet needs of patients with complex genetic disorders like 1p36 deletion syndrome. Off-label use of melatonin (for insomnia) and methylphenidate (for attention deficit/hyperactivity disorder) should be considered in young children with 1p36 deletion syndrome if behavioral interventions are not sufficient.
At present, there is a lack of longitudinal studies on the psychological adjustment of both children and adolescents with 22q11.2 deletion syndrome (22q11.2DS) and their primary caregivers. To fill this gap, we performed a four-year follow-up study. Mothers filled out the Child Behavior Checklist 4–18, the Social Orientation of Parents with Handicapped Children questionnaire to assess maternal stress and coping strategies, and the Freiburger Personality Inventory-Revised — subscales strain and life satisfaction. Fifty-five subjects with 22q11.2DS (26 males and 29 females; age: M = 10.79 years, SD = 3.56 years) and their biological mothers (age: M = 40.84 years, SD = 4.68 years) were included in this study. Significantly higher levels of behavior problems than in the general population and an increase in these problems, especially internalizing ones, over time could be found. In contrast, maternal stress did not change significantly over time, but mothers demonstrated increased levels of strain and reduced life satisfaction at T2. Thus, careful monitoring as well as early and adequate interventions, if indicated, should be offered to families with a child with 22q11.2DS, not only for somatic complaints but also for problems with psychological adjustment.
Background
The psychometric properties of an instrument, the Activity Scale for Kids-performance (ASKp), were assessed which was proposed to capture physical functioning after allogeneic hematopoietic stem cell transplantation (HSCT). Additionally, this multicenter observational prospective study investigated the influence of clinical correlates focusing on chronic graft-versus-host disease (cGVHD).
Methods
Patient-reported ASKp, clinician-reported Karnofsky/Lansky status (KPS/PSS), patient characteristics and cGVHD details were assessed of 55 patients with a median age of 12 years at baseline after day +100 post-HSCT and every 3 months during the next 18 months. The psychometric properties were evaluated and ASKp and KPS/PSS status was compared using ANOVAS and multiple regression models.
Results
The German version of the ASKp showed good psychometric properties except for ceiling effects. Discrimination ability of the ASKp was good regarding the need for devices but failed to predict cGVHD patients. Both the ASKp and the KPS/PSS were associated with patients after adoptive cell therapy being in need for devices, suffering from overlap cGVHD and from steroid side effects but not with patients’ age and gender. In contrast to the KPS/PSS the ASKp only showed significant differences after merging moderate and severe cGHVD patients when comparing them to No-cGVHD (F = 4.050; p = 0.049), being outperformed by the KPS/PSS (F = 20.082; p < 0.001).
Conclusion
The ASKp showed no clear advantages compared to KPS/PSS even though economical and patients’ effort was higher. Further application range may be limited through ceiling effects. Both should be taken into consideration. Therefore, the results may not support the usage of ASKp after HSCT and rather suggest KPS/PSS, both patient and clinician reported.
Ziel der vorliegenden Arbeit war es, eine psychometrische Analyse des von W. Briegel entwickelten Fragebogens durchzuführen , der die Zufriedenheit mit dem Aufklärungsgespräch über eine medikamentöse Therapie multiperspektivisch aus Sicht der Beteiligten erfassen soll.
An einer Stichprobe von 63 gültigen Fällen erfolgte eine Evaluation des Fragebogens im Sinne einer Itemanalyse, sowie die Überprüfung auf Validität und Reliabilität. Zudem wurde untersucht ob im Allgemeinen Zufriedenheit angegeben wurden und ob Einflussfaktoren auf das Antwortverhalten und die Zufriedenheit festgestellt werden konnten
Insgesamt wurden die Fragen von allen Beteiligten im Sinne von Zufriedenheit beantwortet. Die Itemanalyse erbrachte hohe Schwierigkeitsindices, hohe Trennschärfewerte und geringe Varianzen. Des Weiteren zeigte der Fragebogen gute Werte für die Interne Konsistenz und für die Prüfung auf Kriteriumsvalidität.
Aufgrund der sich ergebenden Hinweise, dass die hohen Werte der Zufriedenheitsbefragung durch die Konstruktionsweise des Fragebogens zustande gekommen sein könnten, erscheint es sinnvoll, die Ratingskala des Fragebogens zu modifizieren. Dieser so veränderte Fragebogen sollte dann an einer heterogeneren Stichprobe erneut eingesetzt werden, um herauszufinden, ob die Ergebnisse in diesem Kontext dieselbe Tendenz zeigen.
Background: In recent years, health care has increasingly become the focus of public interest, politics, health insurance companies, and research. This includes the development of therapeutic concepts that can respond individually to patients' resources in order to improve coping with chronic diseases. Research into psychosocial and biological resilience factors is very important and the basic objective of the present work. I studied patients with fibromyalgia syndrome (FMS), who suffer among others from chronic pain, fatigue, sleep and gastrointestinal problems. This patient cohort is characterized by a pronounced heterogeneity in terms of clinical outcome, degree in disability and coping. FMS has a prevalence of 3 – 8 % in the Western population and has a significant socio-economic impact. Validated psychosocial resilience factors include optimism, humor, coherence, self-efficacy, awareness with one's own resources and the ability to apply them profitably (coping), and a healthy social environment with positive relationships. Studies in patients with cancer revealed religiosity as positive and negative factor on the health outcome, but there is little data on religious aspects of pain resilience. Various genetic polymorphisms and anti-inflammatory cytokines are known as biological resilience factors. Various microRNA (miRNA) were detected to contribute to resilience in the context of stress and psychiatric disorders. Objective: The underlying research question of this work is to understand the factors that make some FMS patients resilient and others not, even though they suffer from the same disease. The long-term aim was to understand mechanisms and influencing factors of resilience to design preventive and resource-oriented therapies for FMS patients. Material and Methods: Three studies examined religious, physiological, biological, and psychosocial factors which may contribute to resilience in FMS patients. Study one combined data of questionnaires, a psychosocial interview, and regression analyses to investigate the relevance of religiosity for coping and resilience. Study two examined variance explaining factors and defined clusters among FMS patients by their differences in coping, pain phenotype and disability. The factor analysis used variables derived from questionnaires and qPCR of cytokines in white blood samples (WBC) of patients and healthy controls. Study three assessed cluster-wise miRNA signatures which may underly differences in behaviour, emotional and physiological disability, and resilience among patient clusters. A cluster-specific speculative model of a miRNA-mediated regulatory cycle was proposed and its potential targets verified by an online tool. Results: The data from the first study revealed a not very religious patient cohort, which was rather ambivalent towards the institution church, but described itself as a believer. The degree of religiosity played a role in the choice of coping strategy but had no effect on psychological parameters or health outcomes. The coping strategy "reinterpretation", which is closely related iv to the religious coping "reappraisal", had the highest influence on FMS related disability. Cognitive active coping strategies such as reappraisal which belongs to religious coping had the highest effect on FMS related disability (resilience) and could be trained by a therapist. Results from the second study showed high variances of all measured cytokines within the patient group and no difference between patient and control group. The high dispersion indicated cluster among patients. Factor analysis extracted four variance-explaining factors named as affective load, coping, pain, and pro-inflammatory cytokines. Psychological factors such as depression were the most decisive factors of everyday stress in life and represented the greatest influence on the variance of the data. Study two identified four clusters with respective differences in the factors and characterized them as poorly adapted (maladaptive), well adapted (adaptive), vulnerable and resilient. Their naming was based on characteristics of both resilience concepts, indicated by patients who were less stress-sensitive and impaired as a personal characteristic and by patients who emerged as more resilient from a learning and adaptive process. The data from the variance analysis suggests that problem- and emotion-focused coping strategies and a more anti-inflammatory cytokine pattern are associated with low impairment and contribute to resilience. Additional favorable factors include low anxiety, acceptance, and persistence. Some cluster-specific intervention proposals were created that combine existing concepts of behavioral and mindfulness therapies with alternative therapies such as vitamin D supplementation and a healthy intestinal flora. The results of the third study revealed lower relative gene expression of miR103a-3p, miR107, and miR130a-3p in the FMS cohort compared to the healthy controls with a large effect size. The adaptive cluster had the highest gene expression of miR103a-3p and tendentially of miR107, which was correlated with the subscale score "physical abuse" of the trauma questionnaire. Further correlations were found in particular with pain catastrophizing and FMS-related disability. MiR103a-3p and miR107 form a miRNA-family. Based on this, we proposed a miR103a/107 regulated model of an adaptive process to stress, inflammation and pain by targeting genetic factors which are included in different anti-inflammatory and stress-regulating pathways. Conclusion: All three studies provide new insights into resilience in FMS patients. Cognitive coping (reappraisal/reinterpretation) plays a central role and thus offers therapeutic targets (reframing in the context of behavioral therapy). Religosity as a resilience factor was only partially valid for our patient cohort. Basically, the use of resource-oriented therapy in large institutions still requires research and interdisciplinary cooperation to create a consensus between the humanities, natural sciences and humanism.
Pulssynchrone Ohrgeräusche – Befunderhebung bei einer standardisiert untersuchten Patientengruppe

Die Arbeit umfasst die Auswertung der klinischen Daten von 108 konsekutiv am Kopf- und Schädelbasis-Zentrum, Klinikum Fulda, zwischen Juli 2013 und September 2015 nachuntersuchten Patienten mit pulssynchronen Ohrgeräuschen. Die Patienten waren gemäß eines einheitlichen, strukturierten und routinemäßigen Diagnose- und Behandlungspfads untersucht worden. Die notwendige Bildgebung umfasste die Computertomographie, die Magnetresonanztomographie sowie die Magnetresonanzangiographie und die Sonographie der Halsgefäße. Eine digitale Subtraktionsangiographie wurde nach strenger Indikationsstellung in ausgewählten Fällen vorgenommen. Ziel war es, zugrundeliegende Ursachen und Diagnosen für das Symptom pulssynchrones Ohrgeräusch festzumachen. Die erhobenen Befunde wurden je nach Plausibilitätsgrad in 4 Gruppen klassifiziert: eindeutig, wahrscheinlich, plausibel, und unklar. Zusätzlich erfolgte die Auswertung der angewandten therapeutischen Behandlungsansätze und ihrer Ergebnisse.
Die häufigsten Ursachen waren ein hochstehender Bulbus der Vena jugularis interna mit 19%, eine Schleife der Arteria cerebelli inferior anterior im inneren Gehörgang mit 15% gefolgt von einer arteriovenösen Fistel mit 10% und Gefäßstenosen mit 9%. Bei ca. 30% unseres Patientenkollektivs konnte keine Quelle für das pulsierende Ohrgeräusch eruiert werden. Bei 29% der Patienten konnte die Ursache als eindeutig klassifiziert werden, bei 25% als plausible Ursache und bei 17% als wahrscheinlich. Bei den insgesamt 14 Patienten, die entweder eine konservative oder eine interventionell/chirurgische Behandlung erhielten, kam es bei 71% zu einer Beseitigung des Ohrgeräusches. 5 von 7 Patienten mit einer a.v. Fistel waren, nach einer erfolgreichen endovaskulären Intervention, postoperativ beschwerdefrei. Eine angioplastische Intervention mittels Stenting führte bei zwei Patienten mit einer ACI Stenose (>70%) zur Beseitigung des Ohrgeräusches. Jeweils ein Patient profitierte von der Behandlung einer Bogengangsdehiszenz, einer intrakraniellen Hypertension sowie einer chirurgischen Sanierung eines Paraganglioms, nach vorgegangener endovaskulärer Embolisation, mit der daraus resultierenden Auslöschung des Ohrgeräusches. Bei den restlichen Patienten persistierte es trotz Behandlung.
Ein pulssynchrones Ohrgeräusch hat viele äußerst unterschiedliche Ursachen. Für eine sachgerechte Evaluation und adäquate Behandlung von Patienten mit pulssynchronem Ohrgeräusch ist eine enge multidisziplinäre Zusammenarbeit unabdingbar. Eine eindeutige Diagnose kann oft nur gestellt werden, wenn alle klinischen sowie alle bildmorphologischen Befunde, die auf einem einheitlichen, routinemäßigen Untersuchungsprotokoll basieren, zusammengetragen und kritisch abgestimmt werden.
In dieser Arbeit wird gezeigt, dass Fractalkin (CX3L1) keine Eisenregulation im Sinne des klassischen IRE/IRP-Systems aufweist. Zusätzlich wird die pathophysiologische Rolle der CX3CL1/CX3CR1-Achse in Megakaryozyten untersucht. Ferner wird die Eisenhomöostase während der megakaryopetischen Differenzierung erforscht.
Detection of metals in different environments with high selectivity and specificity is one of the prerequisites of the fight against environmental pollution with these elements. Pyrenes are well suited for the fluorescence sensing in different media. The applied sensing principle typically relies on the formation of intra- and intermolecular excimers, which is however limiting the sensitivity range due to masking of e. g. quenching effects by the excimer emission. Herein we report a highly selective, structurally rigid chemical sensor based on the monomer fluorescence of pyrene moieties bearing triazole groups. This sensor can quantitatively detect Cu\(^{2+}\), Pb\(^{2+}\) and Hg\(^{2+}\) in organic solvents over a broad concentrations range, even in the presence of ubiquitous ions such as Na\(^{+}\), K\(^{+}\), Ca\(^{2+}\) and Mg\(^{2+}\). The strongly emissive sensor's fluorescence with a long lifetime of 165 ns is quenched by a 1 : 1 complex formation upon addition of metal ions in acetonitrile. Upon addition of a tenfold excess of the metal ion to the sensor, agglomerates with a diameter of about 3 nm are formed. Due to complex interactions in the system, conventional linear correlations are not observed for all concentrations. Therefore, a critical comparison between the conventional Job plot interpretation, the method of Benesi-Hildebrand, and a non-linear fit is presented. The reported system enables the specific and robust sensing of medically and environmentally relevant ions in the health-relevant nM range and could be used e. g. for the monitoring of the respective ions in waste streams.
Background
Even though the international combat against Neglected Tropical Diseases such as schistosomiasis or soil-transmitted helminthiases depends on reliable therapeutics, anthelminthic pharmacovigilance has been neglected on many national African drug markets. Therefore, quality and composition of Albendazole, Mebendazole and Praziquantel locally collected in Burkina Faso, Côte d’Ivoire, Ghana and Tanzania were analysed.
Methods
Samples of 88 different batches were obtained from randomly selected facilities. Sampling took place in Northwest Tanzania, Western Burkina Faso, Southeast Côte d’Ivoire and Southwest Ghana. Visual examination of both packaging and samples was performed according to the WHO ‘Be Aware’ tool. Products were then screened with the GPHF Minilab, consisting of tests of mass uniformity, disintegration times and thin-layer chromatography (TLC). Confirmatory tests were performed according to international pharmacopoeiae, applying assays for dissolution profiles and high-performance liquid chromatography (HPLC).
Findings
Despite minor irregularities, appearance of the products did not hint at falsified medicines. However, 19.6% of the brands collected in Ghana and Tanzania were not officially licensed for sale. Mass uniformity was confirmed in 53 out of 58 brands of tablets. 41 out of 56 products passed disintegration times; 10 out of the 15 failing products did not disintegrate at all. Evaluating TLC results, only 4 out of 83 batches narrowly missed specification limits, 18 batches slightly exceeded them. Not more than 46.3% (31 / 67) of the tablets assayed passed the respective pharmaceutical criteria for dissolution. HPLC findings confirmed TLC results despite shifted specification limits: 10 out of 83 tested batches contained less than 90%, none exceeded 110%.
Conclusion
In the four study countries, no falsified anthelminthic medicine was encountered. The active pharmaceutical ingredient was not found to either exceed or fall below specification limits. Galenic characteristics however, especially dissolution profiles, revealed great deficits.
Introduction
The term primary progressive aphasia (PPA) sums up the non‐fluent (nfv), the semantic (sv), and the logopenic (lv) variant. Up to now, there is only limited data available concerning magnetic resonance imaging volumetry to monitor disease progression.
Methods
Structural brain imaging and an extensive assessment were applied at baseline and up to 4‐year(s) follow‐up in 269 participants. With automated atlas‐based volumetry 56 brain regions were assessed. Atrophy progression served to calculate sample sizes for therapeutic trials.
Results
At baseline highest atrophy appeared in parts of the left frontal lobe for nfvPPA (–17%) and of the left temporal lobe for svPPA (–34%) and lvPPA (–24%). Severest progression within 1‐year follow‐up occurred in the basal ganglia in nfvPPA (–7%), in the hippocampus/amygdala in svPPA (–9%), and in (medial) temporal regions in lvPPA (–6%).
Conclusion
PPA presents as a left‐dominant, mostly gray matter sensitive disease with considerable atrophy at baseline that proceeds variant‐specific.
Forest systems provide crucial ecosystem functions to our environment, such as balancing carbon stocks and influencing the local, regional and global climate. A trend towards an increasing frequency of climate change induced extreme weather events, including drought, is hereby a major challenge for forest management. Within this context, the application of remote sensing data provides a powerful means for fast, operational and inexpensive investigations over large spatial scales and time. This study was dedicated to explore the potential of satellite data in combination with harmonic analyses for quantifying the vegetation response to drought events in German forests. The harmonic modelling method was compared with a z-score standardization approach and correlated against both, meteorological and topographical data. Optical satellite imagery from Landsat and the Moderate Resolution Imaging Spectroradiometer (MODIS) was used in combination with three commonly applied vegetation indices. Highest correlation scores based on the harmonic modelling technique were computed for the 6th harmonic degree. MODIS imagery in combination with the Normalized Difference Vegetation Index (NDVI) generated hereby best results for measuring spectral response to drought conditions. Strongest correlation between remote sensing data and meteorological measures were observed for soil moisture and the self-calibrated Palmer Drought Severity Index (scPDSI). Furthermore, forests regions over sandy soils with pine as the dominant tree type were identified to be particularly vulnerable to drought. In addition, topographical analyses suggested mitigated drought affects along hill slopes. While the proposed approaches provide valuable information about vegetation dynamics as a response to meteorological weather conditions, standardized in-situ measurements over larger spatial scales and related to drought quantification are required for further in-depth quality assessment of the used methods and data.
The aim of this pilot study was to investigate the diagnostic potential of TMEM119 as a useful microglia-specific marker in combination with immunostainings for phagocytic function and infiltrating capacity of monocytes in cases of lethal monosubstance intoxications by morphine (MOR), methamphetamine (METH), and of ethanol-associated death (ETH) respectively. Human brain tissue samples were obtained from forensic autopsies of cases with single substance abuse (MOR, n = 8; ETH, n = 10; METH, n = 9) and then compared to a cohort of cardiovascular fatalities as controls (n = 9). Brain tissue samples of cortex, white matter, and hippocampus were collected and stained immunohistochemically with antibodies against TMEM119, CD68KiM1P, and CCR2. We could document the lowest density of TMEM119-positive cells in MOR deaths with highly significant differences to the control densities in all three regions investigated. In ETH and METH deaths, the expression of TMEM119 was comparable to cell densities in controls. The results indicate that the immunoreaction in brain tissue is different in these groups depending on the drug type used for abuse.

TNFR1 is a crucial regulator of NF‐ĸB‐mediated proinflammatory cell survival responses and programmed cell death (PCD). Deregulation of TNFα‐ and TNFR1‐controlled NF‐ĸB signaling underlies major diseases, like cancer, inflammation, and autoimmune diseases. Therefore, although being routinely used, antagonists of TNFα might also affect TNFR2‐mediated processes, so that alternative approaches to directly antagonize TNFR1 are beneficial. Here, we apply quantitative single‐molecule localization microscopy (SMLM) of TNFR1 in physiologic cellular settings to validate and characterize TNFR1 inhibitory substances, exemplified by the recently described TNFR1 antagonist zafirlukast. Treatment of TNFR1‐mEos2 reconstituted TNFR1/2 knockout mouse embryonic fibroblasts (MEFs) with zafirlukast inhibited both ligand‐independent preligand assembly domain (PLAD)‐mediated TNFR1 dimerization as well as TNFα‐induced TNFR1 oligomerization. In addition, zafirlukast‐mediated inhibition of TNFR1 clustering was accompanied by deregulation of acute and prolonged NF‐ĸB signaling in reconstituted TNFR1‐mEos2 MEFs and human cervical carcinoma cells. These findings reveal the necessity of PLAD‐mediated, ligand‐independent TNFR1 dimerization for NF‐ĸB activation, highlight the PLAD as central regulator of TNFα‐induced TNFR1 oligomerization, and demonstrate that TNFR1‐mEos2 MEFs can be used to investigate TNFR1‐antagonizing compounds employing single‐molecule quantification and functional NF‐ĸB assays at physiologic conditions.
Morbidität und Mortalität der Lebererkrankung im Rahmen der Cystischen Fiborse (Cystic fibrosis liver disease, CFLD) sind vornehmlich von Ausmaß und Progredienz der Leberfibrose abhängig. Aufgrund der fehlenden Genauigkeit der bisherigen diagnostischen Verfahren werden viele der an CF erkrankten Menschen erst in fortgeschrittenen Stadien diagnostiziert. Schwere Komplikationen einer Leberzirrhose treten häufig bereits im Kindesalter auf. Die Quantitative Sonoelastographie, hier die Acoustic Radiation Force Impulse (ARFI)-Elastographie, ist ein vielversprechendes, nicht-invasives und strahlenfreies Verfahren zur Messung der Gewebesteifigkeit.
Anhand dieser retrospektiven, monozentrischen Studie soll die ARFI-Elastographie im Hinblick auf den klinischen Einsatz bei der CFLD-Diagnostik untersucht werden. Es wurde eruiert, ob sich mittels ARFI-Elastographie Rückschlüsse auf eine CFLD und deren Schweregrade ziehen lassen.
Hierfür wurden die ARFI-Messungen verschiedener Lebersegmente von 62 an CF erkrankten und 19 lebergesunden Kindern und Jugendlichen verglichen. Zudem erfolgte die Korrelation der Ergebnisse mit zwei etablierten klinischen Leberfibrose-Scores (APRI, Williams-Score).
Im Patientenkollektiv konnten tendenziell erhöhte Scherwellengeschwindigkeiten, entsprechend einer fibrotischen Aktivität, gemessen werden. Die transkostale Messposition in Segment VII/VIII (TC VII/VIII) erwies sich als zuverlässigste Position zur Differenzierung zwischen einer CF-Hepatopathie und einem gesunden Leberparenchym. Hingegen war das Errechnen von Cut-off Werten zur Graduierung von Fibrosestadien nicht möglich. Auch war keine Korrelation zu Leberfiborsescores feststellbar.
Insgesamt zeigt sich, dass die Diagnosestellung einer CFLD aktuell nur in Kombination mit bisherigen Messmethoden wie der klinischen Untersuchung, der Laboranalytik und der Sonographie möglich ist. Die Interpretation der ARFI-cut-off Werte bleibt aufgrund mangelnder Sensitivität und Spezifität und vor dem Hintergrund der CF-typischen heterogenen Leberpathologie erschwert. Die ARFI-Elastographie kann als zusätzlicher Baustein in der Diagnostik der CFLD, bei unklaren Befundkonstellationen oder zum Therapie-Monitoring herangezogen werden. Um einen klaren klinischen Einsatz in der Routinediagnostik zu definieren bedarf es weiterer großer, multizentrischer und prospektiver Studien.
Simple Summary
FAP-targeted radiotracers, recently introduced in cancer treatment, accumulate in Cancer-Associated Fibroblasts (CAFs). CAFs are present in tumor lesions but do not correspond to genuine cancer cells, although they behave in an abnormal and disease-promoting manner. One of their characteristic features, the expression of the surface protein FAP, can be utilized to discriminate between cancerous and healthy tissues. By the choice of an appropriate radionuclide, FAP-targeted tracers can be used for imaging or therapy in many cancer types. Therefore, the first successful application of FAP-targeted imaging has led to an enormous and growing interest in nuclear medicine and radiopharmacy.
Abstract
Targeting fibroblast activation protein (FAP) in cancer-associated fibroblasts (CAFs) has attracted significant attention in nuclear medicine. Since these cells are present in most cancerous tissues and FAP is rarely expressed in healthy tissues, anti-FAP tracers have a potential as pan-tumor agents. Compared to the standard tumor tracer [\(^{18}\)F]FDG, these tracers show better tumor-to-background ratios (TBR) in many indications. Unlike [\(^{18}\)F]FDG, FAP-targeted tracers do not require exhausting preparations, such as dietary restrictions on the part of the patient, and offer the possibility of radioligand therapy (RLT) in a theragnostic approach. Although a radiolabeled antibody was clinically investigated as early as the 1990s, the breakthrough event for FAP-targeting in nuclear medicine was the introduction and clinical application of the so-called FAPI-tracers in 2018. From then, the development and application of FAP-targeted tracers became hot topics for the radiopharmaceutical and nuclear medicine community, and attracted the interest of pharmaceutical companies. The aim of this review is to provide a comprehensive overview of the development of FAP-targeted radiopharmaceuticals and their application in nuclear medicine.
Das Targeting des MEK-Proteins in Krebszellen führt in der Regel zu einer erworbenen Resistenz gegen MEK-Inhibitoren und zur Aktivierung des überlebenswichtigen Proteins Akt. Da sowohl MEK als auch Akt Clienten des Hsp90-Chaperonsystems sind, untersucht die vorliegende Arbeit die Reaktionen von bestrahlten Lungenkarzinom- (A549) und Glioblastom- (SNB19) Zelllinien auf eine kombinierte MEK- und Hsp90-Hemmung. Unerwarteterweise verbesserte der 24 h vor der Bestrahlung verabreichte MEK-Inhibitor PD184352 das Zellüberleben durch Hochregulation von MEK und Erk1/2, aber auch von Akt. Im Gegensatz dazu reduzierte PD184352, das 1 h vor der Bestrahlung zugegeben wurde, die Expression von Erk stark und regulierte Akt in beiden Zelllinien nicht hoch. Als Ergebnis verstärkte der MEK-Inhibitor die radiosensibilisierende Wirkung des Hsp90-Inhibitors NVP-AUY922 in Glioblastomzellen (SNB19).
Objective
The current article encompasses a literature review and recommendations for radiotherapy in nodal oligorecurrent prostate cancer.
Materials and methods
A literature review focused on studies comparing metastasis-directed stereotactic ablative radiotherapy (SABR) vs. external elective nodal radiotherapy (ENRT) and studies analyzing recurrence patterns after local nodal treatment was performed. The DEGRO Prostate Cancer Expert Panel discussed the results and developed treatment recommendations.
Results
Metastasis-directed radiotherapy results in high local control (often > 90% within a follow-up of 1–2 years) and can be used to improve progression-free survival or defer androgen deprivation therapy (ADT) according to prospective randomized phase II data. Distant progression after involved-node SABR only occurs within a few months in the majority of patients. ENRT improves metastases-free survival rates with increased toxicity in comparison to SABR according to retrospective comparative studies. The majority of nodal recurrences after initial local treatment of pelvic nodal metastasis are detected within the true pelvis and common iliac vessels.
Conclusion
ENRT with or without a boost should be preferred to SABR in pelvic nodal recurrences. In oligometastatic prostate cancer with distant (extrapelvic) nodal recurrences, SABR alone can be performed in selected cases. Application of additional systemic treatments should be based on current guidelines, with ADT as first-line treatment for hormone-sensitive prostate cancer. Only in carefully selected patients can radiotherapy be initially used without additional ADT outside of the current standard recommendations. Results of (randomized) prospective studies are needed for definitive recommendations.
In einer randomisierten und kontrollierten Studie an 57 ambulanten Patienten mit leichter- bis mittelgradiger Depression wurden die Effekte von körperorientierten Therapieverfahren (Affektregulierende Massagetherapie und Progressive Muskelrelaxation) untersucht. Dazu wurden die Teilnehmer in Massagegruppe (MG, n=30) und Kontrollgruppe (KG, n= 27) eingeteilt. Sie erhielten eine Serie von vier wöchentlichen Einzeltherapien in den jeweiligen Verfahren.
Bei jeder Behandlung wurde eine Selbstbeurteilung mittels Visueller Analogskalen durch die Teilnehmer selbst durchgeführt. Außerdem gab es eine zweimalige Fremdbeurteilung mittels standardisierter Fragebögen (HAMD und BRMS), die vor und nach der kompletten Behandlungsserie durchgeführt wurde.
Es wurden zudem Vor- und Abschlussgespräche durchgeführt und schriftlich dokumentiert. In der Selbstbeurteilung mittels VAS zeigten sich signifikante Ergebnisse zugunsten der Affektregulierenden Massagetherapie. Dabei waren die Dimensionen „Innere Unruhe“, „Schmerzen“, „Psychomotorische Hemmung“ und „Negatives Körpergefühl“ besonders beachtenswert. Auch in der Fremdbeurteilung ergaben sich signifikante Veränderungen zugunsten der Affektregulierenden Massagetherapie (HAMD p=0.034, BRMS p=0.041). Die durchgeführten Abschlussgespräche ergänzten und verfestigten diese Beobachtungen. Die statistische Überlegenheit der Affektregulierenden Massagetherapie lässt sich mit neurophysiologischen, psychologischen und humoralen Effekten begründen. Dabei spielen gesteigerte Interozeption, Aktivierung von CT-Afferenzen, sowie eine verbesserte interpersonelle Resonanz und Schwingungsfähigkeit dabei die entscheidende Rolle.
Die Ergebnisse erbringen neue Evidenz, dass Patienten mit leicht- und mittelgradigen Depressionen von der Behandlung mit Affektregulierender Massagetherapie (ARMT) profitieren können.
Rapid and Efficient Gene Editing for Direct Transplantation of Naive Murine Cas9\(^+\) T Cells

Gene editing of primary T cells is a difficult task. However, it is important for research and especially for clinical T-cell transfers. CRISPR/Cas9 is the most powerful gene-editing technique. It has to be applied to cells by either retroviral transduction or electroporation of ribonucleoprotein complexes. Only the latter is possible with resting T cells. Here, we make use of Cas9 transgenic mice and demonstrate nucleofection of pre-stimulated and, importantly, of naive CD3\(^+\) T cells with guideRNA only. This proved to be rapid and efficient with no need of further selection. In the mixture of Cas9\(^+\)CD3\(^+\) T cells, CD4\(^+\) and CD8\(^+\) conventional as well as regulatory T cells were targeted concurrently. IL-7 supported survival and naivety in vitro, but T cells were also transplantable immediately after nucleofection and elicited their function like unprocessed T cells. Accordingly, metabolic reprogramming reached normal levels within days. In a major mismatch model of GvHD, not only ablation of NFATc1 and/or NFATc2, but also of the NFAT-target gene IRF4 in naïve primary murine Cas9\(^+\)CD3\(^+\) T cells by gRNA-only nucleofection ameliorated GvHD. However, pre-activated murine T cells could not achieve long-term protection from GvHD upon single NFATc1 or NFATc2 knockout. This emphasizes the necessity of gene-editing and transferring unstimulated human T cells during allogenic hematopoietic stem cell transplantation.
Plant stress signalling involves bursts of reactive oxygen species (ROS), which can be mimicked by the application of acute pulses of ozone. Such ozone-pulses inhibit photosynthesis and trigger stomatal closure in a few minutes, but the signalling that underlies these responses remains largely unknown.
We measured changes in Arabidopsis thaliana gas exchange after treatment with acute pulses of ozone and set up a system for simultaneous measurement of membrane potential and cytosolic calcium with the fluorescent reporter R-GECO1.
We show that within 1 min, prior to stomatal closure, O\(_{3}\) triggered a drop in whole-plant CO\(_{2}\) uptake. Within this early phase, O\(_{3}\) pulses (200–1000 ppb) elicited simultaneous membrane depolarization and cytosolic calcium increase, whereas these pulses had no long-term effect on either stomatal conductance or photosynthesis. In contrast, pulses of 5000 ppb O\(_{3}\) induced cell death, systemic Ca\(^{2+}\) signals and an irreversible drop in stomatal conductance and photosynthetic capacity.
We conclude that mesophyll cells respond to ozone in a few seconds by distinct pattern of plasma membrane depolarizations accompanied by an increase in the cytosolic calcium ion (Ca\(^{2+}\)) level. These responses became systemic only at very high ozone concentrations. Thus, plants have rapid mechanism to sense and discriminate the strength of ozone signals.
Chapter 1 deals with the reaction of [Rh(acac)(PMe3)2] with para-substituted 1,4-diphenylbuta-1,3-diynes at room temperature, in which a complex containing a bidentate organic fulvene moiety, composed of two diynes, σ-bound to the rhodium center is formed in an all-carbon [3+2] type cyclization reaction. In addition, a complex containing an organic indene moiety, composed of three diynes, attached to the rhodium center in a bis-σ-manner is formed in a [3+2+3] cyclization process.
Reactions at 100 °C reveal that the third diyne inserts between the rhodium center and the bis-σ-bound organic fulvene moiety. Furthermore, the formation of a 2,5- and a 2,4-bis(arylethynyl)rhodacyclopentadiene is observed. The unique [3+2] cyclization product was used for the synthesis of a highly conjugated organic molecule, which is hard to access or even inaccessible by conventional methods. Thus, at elevated temperatures, reaction of the [3+2] product with para-tolyl isocyanate led to the formation of a purple organic compound containing the organic fulvene structure and one equivalent of para-tolyl isocyanate.
The blue and green [3+2+3] complexes show an unusually broad absorption from 500 – 1000 nm with extinction coefficients ε of up to 11000 M-1 cm-1. The purple organic molecule shows an absorption spectrum similar to those of known diketopyrrolopyrroles.
Additionally, the reaction of [Rh(acac)(PMe3)2] with para-tolyl isocyanate was investigated. A cis-phosphine complex of the form cis-[Rh(acac)(PMe3)2(isocyanate)2] with an isocyanate dimer bound to the rhodium center by one carbon and one oxygen atom was isolated.
Replacing the trimethylphosphine ligands in [Rh(acac)(PMe3)2] with the stronger σ-donating NHC ligand Me2Im (1,3-dimethylimidazolin-2-ylidene), again, drastically alters the reaction. Similar [3+2] and [3+2+3] products to those discussed above could not be unambiguously assigned, but cis- and trans-π-complexes, which are in an equilibrium with the two starting materials, were formed.
Chapters 2 is about the influence of the backbone of the α,ω-diynes on the formation and photophysical properties of 2,5-bis(aryl)rhodacyclopentadienes. Therefore, different α,ω-diynes were reacted with [Rh(acac)(PMe3)2] and [Rh(acac)(P(p-tolyl)3)2] in equimolar amounts. In general, a faster consumption of the rhodium(I) starting material is observed while using preorganized α,ω-diynes with electron withdrawing substituents in the backbone. The isolated PMe3-substituted rhodacyclopentadienes exhibit fluorescence, despite the presence of the heavy atom rhodium, with lifetimes τF of < 1 ns and photoluminescence quantum yields Φ of < 0.01 as in previously reported P(p-tolyl)-substituted 2,5-bis(arylethynyl)rhodacyclopentadienes. However, an isolated P(p-tolyl)-substituted 2,5-bis(aryl)rhodacyclopentadiene shows multiple lifetimes and different absorption and excitation spectra leading to the conclusion that different species may be present.
Reaction of [Rh(acac)(Me2Im)2] with dimethyl 4,4'-(naphthalene-1,8-diylbis(ethyne-2,1-diyl))dibenzoate, results in the formation of a mixture trans- and cis-NHC-substituted 2,5-bis(aryl)rhodacyclopentadienes.
In chapter 3 the reaction of various acac- and diethyldithiocarbamate-substituted rhodium(I) catalysts bearing (chelating)phosphines with α,ω-bis(arylethynyl)alkanes (α,ω-diynes), yielding luminescent dimers and trimers, is described. The photophysical properties of dimers and trimers of the α,ω-diynes were investigated and compared to para-terphenyl, showing a lower quantum yield and a larger apparent Stokes shift.
Furthermore, a bimetallic rhodium(I) complex of the form [Rh2(ox)(P(p-tolyl)3)4] (ox: oxalate) was reacted with a CO2Me-substituted α,ω-tetrayne forming a complex in which only one rhodium(I) center reacts with the α,ω-tetrayne. The photophysical properties of this mixed rhodium(I)/(III) species shows only negligible differences compared to the P(p-tolyl)- and CO2Me-substituted 2,5-bis(arylethynyl)rhodacyclopentadiene, previously synthesized by Marder and co-workers.
The reactions of carbodiimides with the iron arylborylene complex [Fe=BDur(CO)\(_{3}\)(PMe\(_{3}\))] (Dur=2,3,5,6-Me\(_{4}\)C\(_{6}\)H) and the iron bis(borylene) complex [Fe{=BDur}{=BN(SiMe\(_{3}\))\(_{2}\)}(CO)\(_{3}\)] yield a wide variety of temperature-dependent products, including known FeBNC and novel FeBNB metallacycles, complexes of N-heterocyclic boracarbene and spiro-boracarbene ligands and a unique 1,3,2,4-diazadiborolyl pianostool complex, characterized by NMR spectroscopy and X-ray crystallography. The product distributions can be rationalized by considering sequences of cycloaddition, metathesis, insertion, and C−H activation pathways mainly governed by sterics.
Die vorliegende Arbeit behandelt im ersten Abschnitt die Synthese und Reaktivität neuartiger Diborane(4). Ebenfalls wurde die Reaktivität von Dihalogendiboranen(4) gegenüber Phenylazid untersucht, wobei symmetrische Vertreter unter Beibehalt der B-B-Bindung die fünfgliedrigen B2N3 Heterocyclen 14 und 15 lieferten. Der zweite Abschnitt dieser Arbeit beschäftigt sich mit der unerwarteten Reaktivität der NHC-stabilisierten μ-Hydridodiborane(5) XXIII und XXIV. Der abschließende Teil dieser Arbeit befasst sich mit den ersten Versuchen zur Darstellung eines CAAC-stabilisierten, Diboranyl-substituierten Borylens.
Background: Clinical practice guidelines for patients with primary biliary cholangitis (PBC) have been recently revised and implemented for well-established response criteria to standard first-line ursodeoxycholic acid (UDCA) therapy at 12 months after treatment initiation for the early identification of high-risk patients with inadequate treatment responses who may require treatment modification. However, there are only very limited data concerning the real-world clinical management of patients with PBC in Germany. Objective: The aim of this retrospective multicenter study was to evaluate response rates to standard first-line UDCA therapy and subsequent Second-line treatment regimens in a large cohort of well-characterized patients with PBC from 10 independent hepatological referral centers in Germany prior to the introduction of obeticholic acid as a licensed second-line treatment option. Methods: Diagnostic confirmation of PBC, standard first-line UDCA treatment regimens and response rates at 12 months according to Paris-I, Paris-II, and Barcelona criteria, the follow-up cut-off alkaline phosphatase (ALP) ≤ 1.67 × upper limit of normal (ULN) and the normalization of bilirubin (bilirubin ≤ 1 × ULN) were retrospectively examined between June 1986 and March 2017. The management and hitherto applied second-line treatment regimens in patients with an inadequate response to UDCA and subsequent response rates at 12 months were also evaluated. Results: Overall, 480 PBC patients were included in this study. The median UDCA dosage was 13.2 mg UDCA/kg bodyweight (BW)/d. Adequate UDCA treatment response rates according to Paris-I, Paris-II, and Barcelona criteria were observed in 91, 71.3, and 61.3% of patients, respectively. In 83.8% of patients, ALP ≤ 1.67 × ULN were achieved. A total of 116 patients (24.2%) showed an inadequate response to UDCA according to at least one criterion. The diverse second-line treatment regimens applied led to significantly higher response rates according to Paris-II (35 vs. 60%, p = 0.005), Barcelona (13 vs. 34%, p = 0.0005), ALP ≤ 1.67 × ULN and bilirubin ≤ 1 × ULN (52.1 vs. 75%, p = 0.002). The addition of bezafibrates appeared to induce the strongest beneficial effect in this cohort (Paris II: 24 vs. 74%, p = 0.004; Barcelona: 50 vs. 84%, p = 0.046; ALP < 1.67 × ULN and bilirubin ≤ 1 × ULN: 33 vs. 86%, p = 0.001). Conclusion: Our large retrospective multicenter study confirms high response rates following UDCA first-line standard treatment in patients with PBC and highlights the need for close monitoring and early treatment modification in high-risk patients with an insufficient response to UDCA since early treatment modification significantly increases subsequent response rates of these patients.
Catastrophizing thoughts may contribute to the development of anxiety, but functional emotion regulation may help to improve treatment. No study so far directly compared up- and down-regulation of fear by cognitive reappraisal. Here, healthy individuals took part in a cued fear experiment, in which multiple pictures of faces were paired twice with an unpleasant scream or presented as safety stimuli. Participants (N = 47) were asked (within-subjects) to down-regulate, to up-regulate and to maintain their natural emotional response. Valence and arousal ratings indicated successful up- and down-regulation of the emotional experience, while heart rate and pupil dilation increased during up-regulation, but showed no reduction in down-regulation. State and trait anxiety correlated with evaluations of safety but not threat stimuli, which supports the role of deficient safety learning in anxiety. Reappraisal did not modulate this effect. In conclusion, this study reveals evidence for up-regulation effects in fear, which might be even more efficient than down-regulation on a physiological level and highlights the importance of catastrophizing thoughts for the maintenance of fear and anxiety.
With the exception of a few signaling incompetent decoy receptors, the receptors of the tumor necrosis factor receptor superfamily (TNFRSF) are signaling competent and engage in signaling pathways resulting in inflammation, proliferation, differentiation, and cell migration and also in cell death induction. TNFRSF receptors (TNFRs) become activated by ligands of the TNF superfamily (TNFSF). TNFSF ligands (TNFLs) occur as trimeric type II transmembrane proteins but often also as soluble ligand trimers released from the membrane-bound form by proteolysis. The signaling competent TNFRs are efficiently activated by the membrane-bound TNFLs. The latter recruit three TNFR molecules, but there is growing evidence that this is not sufficient to trigger all aspects of TNFR signaling; rather, the formed trimeric TNFL–TNFR complexes have to cluster secondarily in the cell-to-cell contact zone for full TNFR activation. With respect to their response to soluble ligand trimers, the signaling competent TNFRs can be subdivided into two groups. TNFRs of one group, designated as category I TNFRs, are robustly activated by soluble ligand trimers. The receptors of a second group (category II TNFRs), however, failed to become properly activated by soluble ligand trimers despite high affinity binding. The limited responsiveness of category II TNFRs to soluble TNFLs can be overcome by physical linkage of two or more soluble ligand trimers or, alternatively, by anchoring the soluble ligand molecules to the cell surface or extracellular matrix. This suggests that category II TNFRs have a limited ability to promote clustering of trimeric TNFL–TNFR complexes outside the context of cell–cell contacts. In this review, we will focus on three aspects on the relevance of receptor oligomerization for TNFR signaling: (i) the structural factors which promote clustering of free and liganded TNFRs, (ii) the signaling pathway specificity of the receptor oligomerization requirement, and (iii) the consequences for the design and development of TNFR agonists.
Objectives: Deep brain stimulation (DBS) programming is based on clinical response testing. Our clinical pilot trial assessed the feasibility of image-guided programing using software depicting the lead location in a patient-specific anatomical model.
Methods: Parkinson's disease patients with subthalamic nucleus-DBS were randomly assigned to standard clinical-based programming (CBP) or anatomical-based (imaging-guided) programming (ABP) in an 8-week crossover trial. Programming characteristics and clinical outcomes were evaluated.
Results: In 10 patients, both programs led to similar motor symptom control (MDS-UPDRS III) after 4 weeks (medicationOFF/stimulationON; CPB: 18.27 ± 9.23; ABP: 18.37 ± 6.66). Stimulation settings were not significantly different, apart from higher frequency in the baseline program than CBP (p = 0.01) or ABP (p = 0.003). Time spent in a program was not significantly different (CBP: 86.1 ± 29.82%, ABP: 88.6 ± 29.0%). Programing time was significantly shorter (p = 0.039) with ABP (19.78 ± 5.86 min) than CBP (45.22 ± 18.32).
Conclusion: Image-guided DBS programming in PD patients drastically reduces programming time without compromising symptom control and patient satisfaction in this small feasibility trial.
Reduced Recombination Losses in Evaporated Perovskite Solar Cells by Postfabrication Treatment

The photovoltaic perovskite research community has now developed a large set of tools and techniques to improve the power conversion efficiency (PCE). One such arcane trick is to allow the finished devices to dwell in time, and the PCE often improves. Herein, a mild postannealing procedure is implemented on coevaporated perovskite solar cells confirming a substantial PCE improvement, mainly attributed to an increased open-circuit voltage (V\(_{OC}\)). From a V\(_{OC}\) of around 1.11 V directly after preparation, the voltage improves to more than 1.18 V by temporal and thermal annealing. To clarify the origin of this annealing effect, an in-depth device experimental and simulation characterization is conducted. A simultaneous reduction of the dark saturation current, the ideality factor (n\(_{id}\)), and the leakage current is revealed, signifying a substantial impact of the postannealing procedure on recombination losses. To investigate the carrier dynamics in more detail, a set of transient optoelectrical methods is first evaluated, ascertaining that the bulk carrier lifetime is increased with device annealing. Second, a drift-diffusion simulation is used, confirming that the beneficial effect of the annealing has its origin in effective bulk trap passivation that accordingly leads to a reduction of Shockley–Read–Hall recombination rates.
Anxiety patients over-generalize fear, possibly because of an incapacity to discriminate threat and safety signals. Discrimination trainings are promising approaches for reducing such fear over-generalization. Here we investigated the efficacy of a fear-relevant vs. a fear-irrelevant discrimination training on fear generalization and whether the effects are increased with feedback during training. Eighty participants underwent two fear acquisition blocks, during which one face (conditioned stimulus, CS+), but not another face (CS−), was associated with a female scream (unconditioned stimulus, US). During two generalization blocks, both CSs plus four morphs (generalization stimuli, GS1–GS4) were presented. Between these generalization blocks, half of the participants underwent a fear-relevant discrimination training (discrimination between CS+ and the other faces) with or without feedback and the other half a fear-irrelevant discrimination training (discrimination between the width of lines) with or without feedback. US expectancy, arousal, valence ratings, and skin conductance responses (SCR) indicated successful fear acquisition. Importantly, fear-relevant vs. fear-irrelevant discrimination trainings and feedback vs. no feedback reduced generalization as reflected in US expectancy ratings independently from one another. No effects of training condition were found for arousal and valence ratings or SCR. In summary, this is a first indication that fear-relevant discrimination training and feedback can improve the discrimination between threat and safety signals in healthy individuals, at least for learning-related evaluations, but not evaluations of valence or (physiological) arousal.
Background: Urinary tract infections (UTIs) are a common cause of prescribing antibiotics in family medicine. In Germany, about 40% of UTI-related prescriptions are second-line antibiotics, which contributes to emerging resistance rates. To achieve a change in the prescribing behaviour among family physicians (FPs), this trial aims to implement the guideline recommendations in German family medicine.
Methods/design: In a randomized controlled trial, a multimodal intervention will be developed and tested in family practices in four regions across Germany. The intervention will consist of three elements: information on guideline recommendations, information on regional resistance and feedback of prescribing behaviour for FPs on a quarterly basis. The effect of the intervention will be compared to usual practice. The primary endpoint is the absolute difference in the mean of prescribing rates of second-line antibiotics among the intervention and the control group after 12 months. To detect a 10% absolute difference in the prescribing rate after one year, with a significance level of 5% and a power of 86%, a sample size of 57 practices per group will be needed. Assuming a dropout rate of 10%, an overall number of 128 practices will be required. The accompanying process evaluation will provide information on feasibility and acceptance of the intervention.
Discussion: If proven effective and feasible, the components of the intervention can improve adherence to antibiotic prescribing guidelines and contribute to antimicrobial stewardship in ambulatory care.

Weiterführende Reduktion des entstandenen Komplexes liefert ein hoch nukleophiles (Boranyliden)methanolat.
The ability to efficiently recognize the emotions on others’ faces is something that most of us take for granted. Children with callous-unemotional (CU) traits and impulsivity/conduct problems (ICP), such as attention-deficit hyperactivity disorder, have been previously described as being “fear blind”. This is also associated with looking less at the eye regions of fearful faces, which are highly diagnostic. Previous attempts to intervene into emotion recognition strategies have not had lasting effects on participants’ fear recognition abilities. Here we present both (a) additional evidence that there is a two-part causal chain, from personality traits to face recognition strategies using the eyes, then from strategies to rates of recognizing fear in others; and (b) a pilot intervention that had persistent effects for weeks after the end of instruction. Further, the intervention led to more change in those with the highest CU traits. This both clarifies the specific mechanisms linking personality to emotion recognition and shows that the process is fundamentally malleable. It is possible that such training could promote empathy and reduce the rates of antisocial behavior in specific populations in the future.
Ziel dieser Arbeit war es, den Einfluss von IL-12 auf die Rezeptoren DNAM-1, TIGIT und CD96 auf NK-Zellen näher zu untersuchen. Wir konnten nachweisen, dass IL-12 den Rezeptor DNAM-1 hochreguliert. CD96 wurde nach 48-stündiger Inkubation mit IL-12 bei frisch isolierten NK-Zellen zunächst ebenfalls hochreguliert, nach längerer in vitro Kultur mit IL-2/IL-15 und anschließender Intervention mit IL-12 für 48h fiel CD96 allerdings wieder unter das Ausgangsniveau ab. Die höchste Steigerung der Expression durch IL-12 konnte an den Rezeptoren CD62L (Adhäsion) und CD161 (Inhibition) beobachtet werden.
Die Ergebnisse wiesen darauf hin, dass IL-12 einen Einfluss auf das Verhältnis der NK-Subpopulationen besitzt, indem es durch Hochregulation von CD56 und Herabregulation von CD16 zu einer Umwandlung von CD56dimCD16+ NK-Zellen zu CD56brightCD16- NK-Zellen beitrug. Während bei beiden Populationen DNAM-1 hochreguliert wurde, stieg die Expression von CD96 in der CD56dim Population, fiel aber in der CD56bright Population. Die Expression von TIGIT verhielt sich in der IL-15 Gruppe gegensätzlich dazu.
IFN-γ konnte die Expression der Liganden für DNAM-1, TIGIT und CD96 auf einer der untersuchten Tumorzelllinien (SK-ES-1) steigern.
Die Zytotoxizität von NK-Zellen konnte nicht durch IL-12 gesteigert werden. Indessen konnten wir feststellen, dass DNAM-1 für die Aufrechterhaltung der zytotoxischen Funktion essentiell war und eine Blockierung von DNAM-1 zu einer drastischen Verringerung derselben geführt hat. Dagegen konnten die NK-Zellen ihre Funktion nach der Blockierung von CD96 weitestgehend aufrechterhalten, es kam allerdings auch nicht zu einer gesteigerten Lyse von Tumorzellen.
Die Ergebnisse verdeutlichten, dass IL-12 zwar die Expression von DNAM-1 auf NK-Zellen zu steigern vermochte, dies allerdings nicht zu einer gesteigerten Zytotoxizität der NK-Zellen gegenüber den getesteten drei Tumorzelllinien geführt hat.
Rekrutierung von Stromazellen aus gefäßwandresidenten Vorläuferzellen während der Tumorgenese

Tumore bestehen nicht nur aus malignen Zellen, sondern ebenfalls aus einer Vielzahl an nicht tumorigenen Zellen, die den Tumor auf vielfältige Weise unterstützen und den Tumor vor therapeutischen Maßnahmen schützen. Die Frage der Herkunft dieser Zellen insbesondere in einem nicht vaskularisierten Tumor ist daher auch für die Entwicklung zukünftiger Therapeutika relevant. In dieser Arbeit wurde eine Methode etabliert, die im dreidimensionalen Raum die Untersuchung des Einflusses von Tumorzellen auf die vaskuläre Adventitia am Model der Mausaorta ermöglicht. Dazu erfolgte die Einbettung von Alginatbeads aus verschiedenen Tumorzelllinien in eine gemeinsame Kollagenmatrix mit murinen Aortenringen. Während des zehntägigem Versuchszeitraums wurde die Aussprossung von Zellen aus den Aortenringen beobachtet und quantifiziert. Es wurde festgestellt, dass die Auswanderung während des Versuchszeitraums zunimmt und dass die Konfrontation mit der Zytokinmischung der Tumorzellen zu einer stärkeren Aussprossung führt, als die Stimulation mit VEGF oder keine Stimulation. Eine gerichtete Auswanderung der Zellen in Richtung der Tumorbeads konnte nicht nachgewiesen bzw. bestätigt werden. Kapilläre Aussprossungen waren nur in geringem Ausmaß zu beobachten. Bei Charakterisierung der ausgewanderten Zellen mittels immunhistochemischer Färbungen waren keine F4/80-positiven und nur einzelne CD34-positive Zellen zu finden. CD31-positive Endothelzellen stellten die Mehrheit der ausgewanderten Zellen bei Tumorzellkonfrontation. Perizyten, die mit dem Marker NG2 gefärbt wurden, stellten eine Mehrheit der migrierten Zellen bei allen Bedingungen.
Die in dieser Arbeit etablierte Methode des Aortenring-Bead-Konfrontationsassays ermöglicht es, in Echtzeit den Einfluss von Tumorzellen auf die Gefäßwand im dreidimensionalen Raum zu beobachten. Der Aortenring-Bead-Konfrontationsassay bietet eine Vielzahl an Variationsmöglichkeiten und stellt daher eine vielversprechende Möglichkeit dar, die Lücke zwischen zweidimensionalen in vitro-Experimenten und kostenintensiven in vivo-Versuchen zu schließen.
Recently reported insect declines have raised both political and social concern. Although the declines have been attributed to land use and climate change, supporting evidence suffers from low taxonomic resolution, short time series, a focus on local scales, and the collinearity of the identified drivers. In this study, we conducted a systematic assessment of insect populations in southern Germany, which showed that differences in insect biomass and richness are highly context dependent. We found the largest difference in biomass between semi-natural and urban environments (-42%), whereas differences in total richness (-29%) and the richness of threatened species (-56%) were largest from semi-natural to agricultural environments. These results point to urbanization and agriculture as major drivers of decline. We also found that richness and biomass increase monotonously with increasing temperature, independent of habitat. The contrasting patterns of insect biomass and richness question the use of these indicators as mutual surrogates. Our study provides support for the implementation of more comprehensive measures aimed at habitat restoration in order to halt insect declines.
After more than one year of the COVID-19 pandemic, antiviral treatment options against SARS-CoV-2 are still severely limited. High hopes that had initially been placed on antiviral drugs like remdesivir have so far not been fulfilled. While individual case reports provide striking evidence for the clinical efficacy of remdesivir in the right clinical settings, major trials failed to demonstrate this. Here, we highlight and discuss the key findings of these studies and underlying reasons for their failure. We elaborate on how such shortcomings should be prevented in future clinical trials and pandemics. We suggest in conclusion that any novel antiviral agent that enters human trials should first be tested in a post-exposure setting to provide rapid and solid evidence for its clinical efficacy before initiating further time-consuming and costly clinical trials for more advanced disease. In the COVID-19 pandemic this might have established remdesivir early on as an efficient antiviral agent at a more suitable disease stage which would have saved many lives, in particular in large outbreaks within residential care homes.
The boreal winter 2019/2020 was very irregular in Europe. While there was very little snow in Central Europe, the opposite was the case in northern Fenno-Scandia, particularly in the Arctic. The snow cover was more persistent here and its rapid melting led to flooding in many places. Since the last severe spring floods occurred in the region in 2018, this raises the question of whether more frequent occurrences can be expected in the future. To assess the variability of snowmelt related flooding we used snow cover maps (derived from the DLR's Global SnowPack MODIS snow product) and freely available data on runoff, precipitation, and air temperature in eight unregulated river catchment areas. A trend analysis (Mann-Kendall test) was carried out to assess the development of the parameters, and the interdependencies of the parameters were examined with a correlation analysis. Finally, a simple snowmelt runoff model was tested for its applicability to this region. We noticed an extraordinary variability in the duration of snow cover. If this extends well into spring, rapid air temperature increases leads to enhanced thawing. According to the last flood years 2005, 2010, 2018, and 2020, we were able to differentiate between four synoptic flood types based on their special hydrometeorological and snow situation and simulate them with the snowmelt runoff model (SRM).
Background
The spondylodysplastic Ehlers-Danlos subtype (OMIM #130070) is a rare connective tissue disorder characterized by a combination of connective tissue symptoms, skeletal features and short stature. It is caused by variants in genes encoding for enzymes involved in the proteoglycan biosynthesis or for a zinc transporter.
Presentation of cases
We report two brothers with a similar phenotype of short stature, joint hypermobility, distinct craniofacial features, developmental delay and severe hypermetropia indicative for a spondylodysplastic Ehlers-Danlos subtype. One also suffered from a recurrent pneumothorax. Gene panel analysis identified two compound heterozygous variants in the B4GALT7 gene: c.641G > A and c.723 + 4A > G. B4GALT7 encodes for galactosyltransferase I, which is required for the initiation of glycosaminoglycan side chain synthesis of proteoglycans.
Conclusions
This is a first full report on two cases with spondylodysplastic Ehlers-Danlos syndrome and the c.723 + 4A > G variant of B4GALT7. The recurrent pneumothoraces observed in one case expand the variable phenotype of the syndrome.
Background
Reproducibility of reported antibacterial activities of plant extracts has long remained questionable. Although plant-related factors should be well considered in serious pharmacognostic research, they are often not addressed in many research papers. Here we highlight the challenges in reproducing antibacterial activities of plant extracts.
Methods
Plants with reported antibacterial activities of interest were obtained from a literature review. Antibacterial activities against Escherichia coli and Klebsiella pneumoniae were tested using extracts’ solutions in 10% DMSO and acetone. Compositions of working solutions from both solvents were established using LC-MS analysis. Moreover, the availability of details likely to affect reproducibility was evaluated in articles which reported antibacterial activities of studied plants.
Results
Inhibition of bacterial growth at MIC of 256–1024 μg/mL was observed in only 15.4% of identical plant species. These values were 4–16-fold higher than those reported earlier. Further, 18.2% of related plant species had MICs of 128–256 μg/mL. Besides, 29.2% and 95.8% of the extracts were soluble to sparingly soluble in 10% DMSO and acetone, respectively. Extracts’ solutions in both solvents showed similar qualitative compositions, with differing quantities of corresponding phytochemicals. Details regarding seasons and growth state at collection were missing in 65% and 95% of evaluated articles, respectively. Likewise, solvents used to dissolve the extracts were lacking in 30% of the articles, whereas 40% of them used unidentified bacterial isolates.
Conclusion
Reproducibility of previously reported activities from plants’ extracts is a multi-factorial aspect. Thus, collective approaches are necessary in addressing the highlighted challenges.
Crizotinib was a first generation of ALK tyrosine kinase inhibitor approved for the treatment of ALK-positive non-small-cell lung carcinoma (NSCLC) patients. COMPARE and cluster analyses of transcriptomic data of the NCI cell line panel indicated that genes with different cellular functions regulated the sensitivity or resistance of cancer cells to crizotinib. Transcription factor binding motif analyses in gene promoters divulged two transcription factors possibly regulating the expression of these genes, i.e., RXRA and GATA1, which are important for leukemia and erythroid development, respectively. COMPARE analyses also implied that cell lines of various cancer types displayed varying degrees of sensitivity to crizotinib. Unexpectedly, leukemia but not lung cancer cells were the most sensitive cells among the different types of NCI cancer cell lines. Re-examining this result in another panel of cell lines indeed revealed that crizotinib exhibited potent cytotoxicity towards acute myeloid leukemia and multiple myeloma cells. P-glycoprotein-overexpressing CEM/ADR5000 leukemia cells were cross-resistant to crizotinib. NCI-H929 multiple myeloma cells were the most sensitive cells. Hence, we evaluated the mode of action of crizotinib on these cells. Although crizotinib is a TKI, it showed highest correlation rates with DNA topoisomerase II inhibitors and tubulin inhibitors. The altered gene expression profiles after crizotinib treatment predicted several networks, where TOP2A and genes related to cell cycle were downregulated. Cell cycle analyses showed that cells incubated with crizotinib for 24 h accumulated in the G\(_2\)M phase. Crizotinib also increased the number of p-H3(Ser10)-positive NCI-H929 cells illustrating crizotinib's ability to prevent mitotic exit. However, cells accumulated in the sub-G\(_0\)G\(_1\) fraction with longer incubation periods, indicating apoptosis induction. Additionally, crizotinib disassembled the tubulin network of U2OS cells expressing an α-tubulin-GFP fusion protein, preventing migration of cancer cells. This result was verified by in vitro tubulin polymerization assays. In silico molecular docking also revealed a strong binding affinity of crizotinib to the colchicine and Vinca alkaloid binding sites. Taken together, these results demonstrate that crizotinib destabilized microtubules. Additionally, the decatenation assay showed that crizotinib partwise inhibited the catalytic activity of DNA topoisomerase II. In conclusion, crizotinib exerted kinase-independent cytotoxic effects through the dual inhibition of tubulin polymerization and topoisomerase II and might be used to treat not only NSCLC but also multiple myeloma.
Introduction: The calipered kinematically-aligned (KA) total knee arthroplasty (TKA) strives to restore the patient's individual pre-arthritic (i.e., native) posterior tibial slope when retaining the posterior cruciate ligament (PCL). Deviations from the patient's individual pre-arthritic posterior slope tighten and slacken the PCL in flexion that drives tibial rotation, and such a change might compromise passive internal tibial rotation and coupled patellofemoral kinematics. Methods: Twenty-one patients were treated with a calipered KA TKA and a PCL retaining implant with a medial ball-in-socket and a lateral flat articular insert conformity that mimics the native (i.e., healthy) knee. The slope of the tibial resection was set parallel to the medial joint line by adjusting the plane of an angel wing inserted in the tibial guide. Three trial inserts that matched and deviated 2°> and 2°< from the patient's pre-arthritic slope were 3D printed with goniometric markings. The goniometer measured the orientation of the tibia (i.e., trial insert) relative to the femoral component. Results: There was no difference between the radiographic preoperative and postoperative tibial slope (0.7 ± 3.2°, NS). From extension to 90° flexion, the mean passive internal tibial rotation with the pre-arthritic slope insert of 19° was greater than the 15° for the 2°> slope (p < 0.000), and 15° for the 2°< slope (p < 0.000). Discussion: When performing a calipered KA TKA with PCL retention, the correct target for setting the tibial component is the patient's individual pre-arthritic slope within a tolerance of ±2°, as this target resulted in a 15–19° range of internal tibial rotation that is comparable to the 15–18° range reported for the native knee from extension to 90° flexion.
Articular cartilage is an exceptional connective tissue which by a network of fibrillar collagen and glycosaminoglycan (GAG) molecules allows both low- friction articulation and distribution of loads to the subchondral bone (Armiento et al., 2018, Ulrich-Vinther et al., 2003). Because of its very limited ability to self-repair, chondral defects following traumatic injury increase the risk for secondary osteoarthritis (OA) (Muthuri et al., 2011). Still, current OA treatments such as common nonsteroidal anti-inflammatory drugs (NSAIDs) and joint replacement primarily address end-stage symptoms (Tonge et al., 2014). As low-grade inflammation plays a pivotal role in the pathogenesis of OA (Robinson et al., 2016), there is a strong demand for novel therapeutic concepts, such as integrating application of anti-inflammatory agents into cartilage cell- based therapies in order to effectively treat OA affected joints in early disease stages. The polyphenolic phytoalexin resveratrol (RSV), found in the skin of red grapes, berries, and peanuts, has been shown to have effective anti-inflammatory properties (Shen et al., 2012). However, its long-term effects on 3D chondrocyte constructs cultured in an inflammatory environment with regard to tissue quality have remained unexplored so far. Therefore, in this study, pellets made from expanded porcine articular chondrocytes were cultured for 14 days with either the pro-inflammatory cytokine interleukin-1β (IL-1β) (1 - 10 ng/ml) or RSV (50 μM) alone, or a co-treatment with both agents. Constructs treated with chondrocyte medium only served as control. Treatment with IL-1β at 10 ng/ml resulted in a significantly smaller pellet size and reduced DNA content. However, RSV counteracted the IL-1β-induced decrease and significantly enhanced diameter and DNA content. Also, in terms of GAG deposition, treatment with IL-1β at 10 ng/ml resulted in a tremendous depletion of absolute GAG content and GAG/DNA. Again, RSV co-treatment counteracted the inflammatory stimulus and led to a partial recovery of GAG content. Histological analysis utilizing safranin-O staining confirmed these findings. Marked expression of the cartilage-degrading enzyme matrix metalloproteinase 13 (MMP13) was detected in IL-1β-treated pellets, but none upon RSV co- treatment. Moreover, co-treatment of IL-1β-challenged constructs with RSV significantly increased absolute collagen content. However, under non- inflammatory conditions, RSV induced gene expression and protein accumulation of collagen type X, a marker for undesirable hypertrophy. Taken together, in the present thesis, RSV was demonstrated to elicit marked beneficial effects on the extracellular matrix composition of 3D cartilaginous constructs in long-term inflammatory culture in vitro, but also induced hypertrophy under non-inflammatory conditions. Based on these findings, further experiments examining multiple concentrations of RSV under various inflammatory conditions appear desirable concerning potential therapeutic applicability in OA.
Boroles are attracting broad interest for their myriad and diverse applications, including in synthesis, small molecule activation and functional materials. Their properties and reactivity are closely linked to the cyclic conjugated diene system, which has been shown to participate in cycloaddition reactions, such as the Diels-Alder reaction with alkynes. The reaction steps leading to boranorbornadienes, borepins and tricyclic boracyclohexenes from the thermal reaction of boroles with alkynes are seemingly well understood as judged from the literature. Herein, we question the long-established mechanistic picture of pericyclic rearrangements by demonstrating that seven-membered borepins (i. e., heptaphenylborepin and two derivatives substituted with a thienyl and chloride substituent on boron) exist in a dynamic equilibrium with the corresponding bicyclic boranorbornadienes, the direct Diels-Alder products, but are not isolable products from the reactions. Heating gradually converts the isomeric mixtures into fluorescent tricyclic boracyclohexenes, the most stable isomers in the series. Results from mechanistic DFT calculations reveal that the tricyclic compounds derive from the boranorbornadienes and not the borepins, which were previously believed to be intermediates in purely pericyclic processes.
Despite the widespread application of landslide susceptibility analyses, there is hardly any information about whether or not the occurrence of recent landslide events was correctly predicted by the relevant susceptibility maps. Hence, the objective of this study is to evaluate four landslide susceptibility maps retrospectively in a landslide-prone area of the Swabian Alb (Germany). The predictive performance of each susceptibility map is evaluated based on a landslide event triggered by heavy rainfalls in the year 2013. The retrospective evaluation revealed significant variations in the predictive accuracy of the analyzed studies. Both completely erroneous as well as very precise predictions were observed. These differences are less attributed to the applied statistical method and more to the quality and comprehensiveness of the used input data. Furthermore, a literature review of 50 peer-reviewed articles showed that most landslide susceptibility analyses achieve very high validation scores. 73% of the analyzed studies achieved an area under curve (AUC) value of at least 80%. These high validation scores, however, do not reflect the high uncertainty in statistical susceptibility analysis. Thus, the quality assessment of landslide susceptibility maps should not only comprise an index-based, quantitative validation, but also an additional qualitative plausibility check considering local geomorphological characteristics and local landslide mechanisms. Finally, the proposed retrospective evaluation approach cannot only help to assess the quality of susceptibility maps and demonstrate the reliability of such statistical methods, but also identify issues that will enable the susceptibility maps to be improved in the future.
Das Ziel der vorliegenden Arbeit war die retrospektive Analyse von Patienten mit einer mykobakteriellen Hautinfektion, die sich an der Klinik und Poliklinik für Dermatologie, Venerologie und Allergologie des Universitätsklinikums Würzburg oder in der Fachabteilung für Tropenmedizin der Missioklinik Würzburg in dem Zeitraum zwischen 1998 und 2018 vorgestellt haben. Es wurden verschiedene Aspekte wie die Anamnese, der mutmaßliche Infektionsweg, klinische und diagnostische Daten sowie die durchgeführte Therapie untersucht. Es konnten insgesamt 32 Fälle erfasst werden, die in 4 erregerspezifische Gruppen unterteilt wurden. Die Gruppe der Mycobacterium (M.) marinum-Infektionen war mit 18 Fällen am größten, gefolgt von 9 Infektionen mit weiteren nichttuberkulösen Mykobakterien (NTM; M. chelonae, M. avium, M. fortuitum, M. wolinskyi/ mageritense sowie M. haemophilum), 3 Fällen mit dem Erreger M. leprae sowie 2 kutanen M. tuberculosis-Infektionen. Der Anteil immunsupprimierter Patienten mit einer M. marinum-Infektion lag bei 22 %. In unserer Untersuchung stellte der Umgang mit Zierfischaquarien den größten Risikofaktor für eine Infektion mit M. marinum dar. Wie unsere Studie weiterhin zeigte, sollte auch bei knotigen Hautveränderungen in für kutane Mykobakteriosen untypischen Körperarealen, wie z.B. dem Gesicht, eine solche Infektion differentialdiagnostisch erwogen werden. Der kulturelle Nachweis misslang lediglich bei 2 Patienten mit einer M. marinum-Infektion. Die Diagnose konnte in diesen beiden Fällen anhand einer charakteristischen Anamnese, Klinik und Histopathologie gestellt werden. In der vorliegenden Untersuchung erfolgte die Therapie bei allen Patienten mittels einer Antibiose. In der M. marinum-Gruppe erwies sich insbesondere der Wirkstoff Clarithromycin als effektiv. Unter der Gabe von Doxycyclin kam es dagegen in 80 % der Fälle zu einer Therapieumstellung. In beiden Gruppen nichttuberkulöser Mykobakteriosen konnten die Monotherapien im Durchschnitt früher erfolgreich beendet werden als die Kombinationstherapien. Letztere kamen allerdings zum Einsatz bei einer zugrundeliegenden Immunsuppression bzw. bei einem ausgedehnten Hautbefund.
Diese retrospektive Analyse untersucht die Behandlung von Patienten mit Alopecia areata in der Klinik und Poliklinik für Dermatologie, Venerologie und Allergologie der Universitätsklinik Würzburg mit Diphenylcyclopropenon und beleuchtet Faktoren der durch die Behandlung beeinflussten Lebensqualität.
In dieser Dissertation wurden die 2-Jahres-Ergebnisse einer FTO mit einer konventionellen TE bei Offenwinkelglaukom miteinander verglichen, um die Effektivität und Sicherheit zu analysieren. Das exact matching, bei dem die Patienten basierend auf dem Ausgangs-IOD und Glaukommedikation 1:1 (FTO:TE) gematcht wurden, ermöglicht eine genaue und ausgewogene Vergleichbarkeit der Daten.
Die FTO wurde bei 110 Patienten durchgeführt. Durch ein verbleibendes Trabeculo-Descemet-Fenster vor dem Sklerallappen konnte eine Iridektomie vermieden werden. Die TE, welche bei 86 Patienten durchgeführt wurde, verwendete eine Block-Exzision des Trabekelmaschenwerks und eine Iridektomie. Bei beiden Operationstechniken wurde MMC verwendet. Die Patienten wurden anhand des Ausgangsaugeninnendrucks (IOD) und der Anzahl der Glaukommedikamente verglichen. Ein absoluter Erfolg (IOD ≤ 18 mmHg und IOD-Reduktion ≥ 30 %, ohne Medikation) und relativer Erfolg (IOD ≤ 18 mmHg und IOD-Reduktion ≥ 30 %, mit Medikation) waren die primären Endpunkte. IOD, Sehschärfe (BCVA), Komplikationen und Interventionen waren sekundäre Endpunkte.
44 FTO-Patienten wurden mit einem IOD von 22,5 ± 4,7 mmHg und 3 ± 0,9 Medikamenten exakt auf 44 TE Patienten abgestimmt. Nach 24 Monaten wurde ein absoluter Erfolg von 59 % bei FTO und 66 % bei TE und ein relativer Erfolg von 59 % bei FTO und 71 % bei TE erzielt. In der FTO-Gruppe reduzierte sich der Augeninnendruck nach 12 Monaten auf 12,4 ± 4,3 mmHg und nach 24 Monaten auf 13,1 ± 4,1 mmHg. In der TE-Gruppe betrug der Augeninnendruck nach 12 Monaten 11,3 ± 2,2 mmHg und nach 24 Monaten 12,0 ± 3.5 mmHg. Die Medikation verringerte sich nach 24 Monaten auf 0,6 ± 1,3 in FTO und 0,2 ± 0,5 in TE.
Zu keinem Zeitpunkt gab es signifikante Unterschiede zwischen den beiden Gruppen in Bezug auf Augeninnendruck, Medikamente, Komplikationen oder weitere Interventionen.
Die Modifikation des Kammerwasserabflusses durch eine begrenzte TE bei FTO führte zu ähnlichen klinischen Ergebnissen wie die herkömmliche Blockexzision des Trabekelmaschenwerks mit Iridektomie bei TE.
Das Ziel der vorliegenden Arbeit war die Untersuchung einer Kohorte erwachsener Patienten mit einseitiger Taubheit (single-sided deafness SSD), die ein Cochlea-Implantat erhalten haben. Es sollten Faktoren ermittelt werden, die es ermöglichen, eine Voraussage über die spätere Hörleistung mit Cochlea Implantat treffen zu können.
Der Beobachtungszeitraum umfasste die Jahre 2009 - 2016. Das Patientenkollektiv bestand aus 55 einseitig ertaubten Erwachsenen. Die Sprachverständlichkeit wurde mit dem Freiburger Zahlentest bei 65 dB SPL, dem Freiburger Einsilbertest bei 65 dB SPL und 80 dB SPL, sowie dem HSM-Satztest ohne Störgeräusch bei 65 dB SPL getestet. Es hat sich gezeigt, dass mit steigender Hörerfahrung mit einer Verbesserung der Hörkompetenz zu rechnen ist. Im untersuchten Patientenkollektiv schnitten Patienten mit der Kombination aus einer langen Ertaubungsdauer und einer entzündlichen Ätiologie signifikant schlechter ab als die Vergleichsgruppe.
Das Neovaskularisationsglaukom (NVG) ist eine gravierende Erkrankung, die zur Erblindung und zum Verlust des Auges führen kann. Es gibt aktuell keine einheitlichen Behandlungsstrategien oder Leitlinien zur Behandlung des NVG. Das Ziel unserer konsekutiven, interventionellen Studie war es Effektivität und Sicherheit eines neuen kombinierten Operationsverfahrens, bestehend aus 23-Gauge-Pars-plana-Vitrektomie, panretinaler, nahezu konfluenter Endolaserkoagulation, transskleraler Zyklophotokoagulation (810-nm-Diodenlaser, 2W, 2s, 20 Herde), intravitrealer Bevacizumab-Eingabe und Lufttamponade retrospektiv zu analysieren. Bei allen phaken Augen wurde intraoperativ eine künstliche Intraokularlinse implantiert. Ausgewertet wurden bestkorrigierte Sehschärfe (BCVA, logMAR), Augeninnendruck (IOD, mmHg), Glaukom-Medikations-Score, visuelle analoge Schmerzskala (VAPS, 0-10) präoperativ und nach 1, 3, 6, und 12 Monaten sowie Komplikationen, erneute Eingriffe und Erfolg. Blinde Augen wurden ausgeschlossen. 83 Augen von 83 Patienten (53 Männer, 30 Frauen; 78 Jahre, SD ± 11,6 Jahre) wurden eingeschlossen. 53 Augen erreichten ein Jahr Nachbeobachtungszeit. Zu den häufigsten Ätiologien des NVG zählten Zentralvenenverschluss (n=41), proliferative diabetische Retinopathie (n=25), Zentralarterienverschluss (n=10) und okuläres Ischämiesyndrom (n=6). Die bestkorrigierte Sehschärfe zeigte keine signifikante Änderung zwischen präoperativ (Mittelwert 2,0; SD ± 0,7 logMAR) und einem Jahr Nachbeobachtung (1,8 ± 0,8 logMAR) (p>0,05, Friedman-Test). Der präoperative IOD (Mittelwert 46,0 ± 10,3 mmHg) sank nach einem Jahr signifikant (14,2 ± 8,9 mmHg) (p<0,001, Friedman-Test). Der Glaukom-Medikations-Score (GMS) nahm zwischen präoperativ (4,8 ± 2,5) und einem Jahr (1,8 ± 1,8) (p<0,001, Friedman-Test) signifikant ab. Präoperativ gaben n=27 (32,5 %) der Patienten auf der visuellen Analogskala (VAS) Schmerzen mit einem Mittelwert von 6,3 ± 1,6 an. Postoperativ waren 96,4 % der Patienten (n=80) schmerzfrei (VAS 0,15 ± 0,5). Patienten mit Schmerzen (49,2 ± 7,9 mmHg) hatten einen signifikant höheren Augeninnendruck als schmerzfreie Patienten (44,5 ± 11,1 mmHg) (p=0,027, Welch-Test). Die postoperative Entzündung war häufig deutlich ausgeprägt, jedoch bei allen Augen vorübergehend und zu jedem Zeitpunkt schmerzlos. Eine Netzhautablösung trat bei insgesamt 4 Augen (6,9 %) auf. 3 von diesen Augen (5,7 %) entwickelten eine Phthisis bulbi.
Die in dieser Dissertation analysierte kombinierte chirurgische Behandlungsstrategie senkt den IOD signifikant, führt zu einer wirksamen Schmerzkontrolle, verringert den Bedarf an Glaukom-Medikamenten und zeigte eine ähnliche Effektivität und Erfolgsrate wie Glaukom-Drainage-Implantate bei vergleichsweise günstigerem Komplikationsspektrum. Der Visus blieb in den meisten Augen unverändert und im Gegensatz zu anderen Studien kam es seltener zu einem Sehverlust. Die Hälfte der Augen benötigten keine weiteren chirurgischen Eingriffe und kein Auge musste enukleiert werden.
Die Exstirpation erkrankter Nebenschilddrüsen (NSD) ist die einzige kurative Therapie des primären Hyperparathyreoidismus (pHPT). Die präoperative Detektion der dem pHPT zugrunde liegenden NSD-Adenome durch eine adäquate Lokalisationsdiagnostik stellt eine wichtige Säule bei der Operationsplanung dar. Angesichts der umfangreichen diagnostischen Möglichkeiten ist noch nicht abschließend beantwortet, wie viel und welche Diagnostik mit hoher Wahrscheinlichkeit zur erfolgreichen Lokalisation von NSD-Adenomen führt und ob/wie diese den perioperativen Verlauf beeinflusst. Die Beantwortung dieser Fragen war das Hauptziel der vorliegenden Arbeit.
Es handelt sich um eine monozentrische, retrospektive Datenanalyse anhand des Kollektivs des Universitätsklinikums Würzburg (UKW) der Jahre 2005 bis 2017. Nach Datenextraktion aller Patienten/Patientinnen mit Hyperparathyreoidismus aus dem Dokumentationssystem des UKW erfolgten die deskriptiven und statistischen Auswertungen mittels Excel und SPSS.
Insgesamt wurden im untersuchten Zeitraum 467 Patienten/Patientinnen aufgrund eines pHPT operiert. NSD-Sono und NSD-Szinti waren die am häufigsten durchgeführten Lokalisationsdiagnostika mit Sensitivitäten von 61,5 % bzw. 66,3 % für die Seite. Bei der Etagen-Blutentnahme lag die Sensitivität bei 100 %; bei der MRT bei 47,4 % und bei der 11Kohlenstoff-Methionin-Positronenemissionstomographie/Computertomographie (11C-Methionin-PET/CT) bei 58,8 %. Durch zusätzliche Diagnostik konnte nicht grundsätzlich eine Erhöhung der Treffsicherheit erreicht werden.
Die Analyse der perioperativen Parameter zeigte, dass das Alter der Operierten positiv mit der Operationsdauer, der Krankenhausaufenthaltsdauer und dem Auftreten postoperativer Hypocalcämien korrelierte. Die Einnahme eines Thrombozytenaggregationshemmers führte zu einer verlängerten Krankenhausaufenthaltsdauer. Die therapeutische Antikoagulation war ein Risikofaktor in Bezug auf längere OP-Dauern und das Auftreten von Nachblutungen. Eine zusätzlich zur Parathyreoidektomie durchgeführte Sanierung der Schilddrüse war mit einer erhöhten Rate an postoperativen Hypocalcämien vergesellschaftet.
Zusammenfassend zeigen die vorliegenden Daten, dass nach initial vermeintlich erfolgreicher Detektion eines NSD-Adenoms mit NSD-Sono oder NSD-Szinti eine weiterführende Lokalisationsdiagnostik nicht sinnvoll ist. Nach initial erfolgloser NSD-Sono oder NSD-Szinti dagegen ist die Durchführung einer 11C-Methionin-PET/CT zu erwägen.
During strong El Niño events, below-average rainfall is expected in large parts of southern Africa. The 1992 El Niño season was associated with one of the worst drought episodes in large parts of South Africa. Using reanalysis data set from NCEP-NCAR, this study examined circulation types (CTs) in Africa south of the equator that are statistically related to the El Niño signal in the southwest Indian Ocean and the implication of this relationship during the 1992 drought episode in South Africa. A statistically significant correlation was found between the above-average Nino 3.4 index and a CT that features widespread cyclonic activity in the tropical southwest Indian Ocean, coupled with a weaker state of the south Indian Ocean high-pressure. During the analysis period, it was found that the El Niño signal enhanced the amplitude of the aforementioned CT. The impacts of the El Niño signal on CTs in southern Africa, which could have contributed to the 1992 severe drought episode in South Africa, were reflected in (i) robust decrease in the frequency of occurrence of the austral summer climatology pattern of atmospheric circulation that favors southeasterly moisture fluxes, advected by the South Indian Ocean high-pressure; (ii) modulation of easterly moisture fluxes, advected by the South Atlantic Ocean high-pressure, ridging south of South Africa; (iii) and enhancement of the amplitude of CTs that both enhances subsidence over South Africa, and associated with the dominance of westerlies across the Agulhas current. Under the ssp585 scenario, the analyzed climate models suggested that the impact of radiative heating on the CT significantly related to El Niño might result in an anomalous increase in surface pressure at the eastern parts of South Africa.
Rezeptor-vermittelte Chemotherapie von ovarialen Karzinomzellen mit Disorazol-GnRH-Konjugaten

Das Ovarialkarzinom stellt einen häufigen maligen Tumor der Frau dar, der meist spät diagnostiziert wird. Therapeutische Optionen sind nur eingeschränkt verfügbar und nebenwirkungsbehaftet. In der modernen Tumortherapie sind zielgerichtete medikamentöse Ansätze von immer größer Bedeutung und sind bei verschiedenen Entitäten bereits zugelassen. Da Ovarialkarzinome häufig GnRH-Rezeptoren exprimieren, stellt dies einen guten Angriffspunkt für mögliche Therapeutika dar.
In dieser Arbeit wurde die Wirkung von Disorazol, einem potenten Zytotoxin, in Kopplung an GnRH auf Ovarialkarzinom-Zellen untersucht. Unter anderem wurden hierbei RT-PCR, Kristallviolettversuche, WST-Versuche und FACS-Analysen durchgeführt.
Molekularbiologisch war eine deutliche Expression von GnRH-Rezeptoren auf ovarialen Karzinomzellen zu sehen. Es zeigte sich eine spezifische Toxizität von GnRH-Disorazol-Konjugaten auf Ovarialkarzinom-Zelllinien und andere GnRH-tragende Zellen. Lymphozyten aus dem peripheren Blut waren nicht im besonderen Maße anfällig für Disorazol. Verapamil konnte in einzelnen Zelllinien die Toxizität des Konjugats verstärken, eine Cisplatin-Resistenz hatte jedoch keinen Einfluss darauf. Apoptose-inhibierende Substanzen wie zVAD verminderten den Anteil an toten Zellen, Necrostatin war dazu nicht in der Lage.
Die spezifische Wirksamkeit von GnrH gekoppeltem Disorazol auf Ovarialkarzinomzellen bestätigt das ursprüngliche Therapiekonzept. Eine ausgeprägtere Hämatotoxizität konnte nicht nachgewiesen werden, was im Hinblick auf den klinischen Einsatz eine bedeutende Rolle spielt. Da einige weitere Entitäten wie das triple-negative Mamma-Karzinom GnRH-Rezeptor-exprimierende Zellen aufweisen, ist ein Einsatz auch in diesen Krankheitsbildern denkbar.
A series of highly substituted 1,2-azaborinines, including a phenylene-bridged bis-1,2-azaborinine, was synthesized from the reaction of 1,2-azaborete rhodium complexes with variously substituted alkynes. 1-Rhoda-3,2-azaborole complexes, which are accessible by phosphine addition to the corresponding 1,2-azaborete complexes, were also found to be suitable precursors for the synthesis of 1,2-azaborinines and readily reacted with alkynyl-substituted 1,2-azaborinines to generate new regioisomers of bi-1,2-azaborinines, which feature directly connected aromatic rings. Their molecular structures, which can be viewed as boron-nitrogen isosteres of biphenyls, show nearly perpendicular 1,2-azaborinine rings. The new method using rhodacycles instead of 1,2-azaborete complexes as precursors is shown to be more effective, allowing the synthesis of a wider range of 1,2-azaborinines.
Both brainstem auditory evoked potentials (BAEP) and audiometry play a crucial role in neuro-oncological treatment decisions in Neurofibromatosis Type 2 associated (NF2) vestibular schwannoma (VS) as hearing preservation is the major goal. In this study, we investigated the risk of immediate postoperative hearing deterioration (>15 dB and/or 15% loss in pure-tone average [PTA]/ speech discrimination score [SDS] in a cohort of 100 operated VS (ears) in 72 NF2 patients by retrospective analysis of pre- and postoperative hearing data (PTA, SDS, American Association of Otolaryngology–Head and Neck Surgery [AAO-HNS], and brainstem auditory evoked potential [BAEP] class) taking into account relevant influencing factors, particularly preoperative audiometry and BAEP status and the extent of resection. Immediately after surgery, the hearing was preserved in 73% of ears and approximately ~60% of ears kept their hearing classes. Preoperative BAEP (p = 0.015) and resection amount (p = 0.048) significantly influenced postoperative hearing outcome. The prediction model for postoperative hearing deterioration/loss between preoperative BAEP and AAO-HNS class showed increased risk by increasing BAEP class. Twenty-one tumors/ears were identified with large BAEP and AAO-HNS class discrepancies (≥2 points) and were associated with a high (48–100%) risk of deafness after surgery in ears with preoperative available hearing. Overall, the results were heterogeneous but the better both BAEP and audiometry class before surgery, the higher the chance of hearing maintenance afterwards. Large resection amounts (e.g., 100% risk in near-total resections) exhibit a significant (p < 0.05) higher risk compared to smaller amounts (e.g., 10/20% in laser-coagulated/partially resected tumors). Our results emphasized the indispensable role of both hearing monitoring in form of audiometry and neurophysiology (BAEP) in the pre-and perioperative monitoring of NF2-associated VS. Both BAEP and audiometry are good prognostic markers for the postoperative hearing outcome. The extent of resection should be strictly guided by and adjusted to the intraoperative neurophysiological monitoring.
Purpose
Rapid accessibility of (intensive) medical care can make the difference between life and death. Initial care in case of strokes is highly dependent on the location of the patient and the traffic situation for supply vehicles. In this methodologically oriented paper we want to determine the inequivalence of the risks in this respect.
Methods
Using GIS we calculate the driving time between Stroke Units in the district of Münster, Germany for the population distribution at day- & nighttime. Eight different speed scenarios are considered. In order to gain the highest possible spatial resolution, we disaggregate reported population counts from administrative units with respect to a variety of factors onto building level.
Results
The overall accessibility of urban areas is better than in less urban districts using the base scenario. In that scenario 6.5% of the population at daytime and 6.8% at nighttime cannot be reached within a 30-min limit for the first care. Assuming a worse traffic situation, which is realistic at daytime, 18.1% of the population fail the proposed limit.
Conclusions
In general, we reveal inequivalence of the risks in case of a stroke depending on locations and times of the day. The ability to drive at high average speeds is a crucial factor in emergency care. Further important factors are the different population distribution at day and night and the locations of health care facilities. With the increasing centralization of hospital locations, rural residents in particular will face a worse accessibility situation.
To generate infectious viral particles, viruses must specifically select their genomic RNA from milieu that contains a complex mixture of cellular or non-genomic viral RNAs. In this review, we focus on the role of viral encoded RNA structures in genome packaging. We first discuss how packaging signals are constructed from local and long-range base pairings within viral genomes, as well as inter-molecular interactions between viral and host RNAs. Then, how genome packaging is regulated by the biophysical properties of RNA. Finally, we examine the impact of RNA packaging signals on viral evolution.
Deoxyribozymes are emerging as modification-specific endonucleases for the analysis of epigenetic RNA modifications. Here, we report RNA-cleaving deoxyribozymes that differentially respond to the presence of natural methylated cytidines, 3-methylcytidine (m\(^3\)C), N\(^4\)-methylcytidine (m\(^4\)C), and 5-methylcytidine (m\(^5\)C), respectively. Using in vitro selection, we found several DNA catalysts, which are selectively activated by only one of the three cytidine isomers, and display 10- to 30-fold accelerated cleavage of their target m\(^3\)C-, m\(^4\)C- or m\(^5\)C-modified RNA. An additional deoxyribozyme is strongly inhibited by any of the three methylcytidines, but effectively cleaves unmodified RNA. The mXC-detecting deoxyribozymes are programmable for the interrogation of natural RNAs of interest, as demonstrated for human mitochondrial tRNAs containing known m\(^3\)C and m\(^5\)C sites. The results underline the potential of synthetic functional DNA to shape highly selective active sites.

Endoscopic management of umbilical and incisional hernias has adapted to the limitations of conventional laparoscopic instruments over the past 30 years. This includes the development of meshes for intraperitoneal placement (intraperitoneal onlay mesh, IPOM), with antiadhesive coatings; however, adhesions do occur in a significant proportion of these patients. Minimally invasive procedures result in fewer perioperative complications, but with a slightly higher recurrence rate. With the ergonomic resources of robotics, which offers angled instruments, it is now possible to implant meshes in a minimally invasively manner in different abdominal wall layers while achieving morphologic and functional reconstruction of the abdominal wall. This video article presents the treatment of ventral and incisional hernias with mesh implantation into the preperitoneal space (robot-assisted transabdominal preperitoneal ventral hernia repair, r‑ventral TAPP) as well as into the retrorectus space (r-Rives and robotic transabdominal retromuscular umbilical prosthetic repair, r‑TARUP, respectively). The results of a cohort study of 118 consecutive patients are presented and discussed with regard to the added value of the robotic technique in extraperitoneal mesh implantation and in the training of residents.
The principle of targeted separation or weakening of individual components of the abdominal wall to relieve tension in the median line during major abdominal reconstruction has been known for over 30 years as anterior component separation (aCS) and is an established procedure. In search of alternatives with lower complication rates, posterior component separation (pCS) was developed; transversus abdominis release (TAR) is a nerve-sparing modification of pCS. With the ergonomic resources of robotics (e.g., angled instruments), TAR can be performed in a minimally invasive manner (r-TAR): hernia gaps of up to 14 cm can be closed and a large extraperitoneal mesh implanted. In this video article, the treatment of large incisional hernias using the r‑TAR technique is presented. Exemplary results of a cohort study in 13 consecutive patients are presented. The procedure is challenging, but our own results—as well as reports from the literature—are encouraging. The r‑TAR is becoming the pinnacle procedure for abdominal wall reconstruction.
The treatment of inguinal hernias with open and minimally invasive procedures has reached a high standard in terms of outcome over the past 30 years. However, there is still need for further improvement, mainly in terms of reduction of postoperative seroma, chronic pain, and recurrence. This video article presents the endoscopic anatomy of the groin with regard to robotic transabdominal preperitoneal patch plasty (r‑TAPP) and illustrates the surgical steps of r‑TAPP with respective video sequences. The results of a cohort study of 302 consecutive hernias operated by r‑TAPP are presented and discussed in light of the added value of the robotic technique, including advantages for surgical training. r‑TAPP is the natural evolution of conventional TAPP and has the potential to become a new standard as equipment availability increases and material costs decrease. Future studies will also have to refine the multifaceted added value of r‑TAPP with new parameters.
Die Versorgung von Leistenhernien mit offenen und minimal-invasiven Verfahren hat in den vergangenen 30 Jahren einen vom Ergebnis her gesehen hohen Standard erreicht. Allerdings besteht noch Bedarf an einer weiteren Reduktion der postoperativen Serome, chronischen Schmerzen und des Rezidivs. In diesem Videobeitrag wird die endoskopische Anatomie der Leiste im Hinblick auf die robotische transabdominelle präperitoneale Patchplastik (r‑TAPP) dargestellt und die Operationsschritte der r‑TAPP als Video illustriert. Es werden die Ergebnisse einer Kohortenstudie an 302 konsekutiven Hernien, die mittels r‑TAPP operiert wurden, vorgestellt und hinsichtlich des Mehrwerts der robotischen Technik – auch in der Weiterbildung – diskutiert. Die r‑TAPP ist die natürliche Weiterentwicklung der konventionellen TAPP und hat das Potenzial, bei zunehmender Geräteverfügbarkeit und Reduktion der Materialkosten zu einem neuen Standard zu werden. Künftige Studien werden den vielseitigen Mehrwert der r‑TAPP auch mit neuen Parametern verfeinern müssen.
Die endoskopische Versorgung von Umbilikal- und Inzisionalhernien hat sich in den vergangenen 30 Jahren an die Limitationen der konventionellen laparoskopischen Instrumente angepasst. Dazu gehört die Entwicklung von Netzen für die intraperitoneale Lage (intraperitoneales Onlay-Mesh, IPOM) mit antiadhäsiven Beschichtungen; allerdings kommt es bei einem beträchtlichen Teil dieser Patienten doch zu Adhäsionen. Minimal-invasive Verfahren führen zu weniger perioperativen Komplikationen, bei einer etwas höheren Rezidivrate. Mit den ergonomischen Ressourcen der Robotik, die abgewinkelte Instrumente anbietet, besteht erstmals die Möglichkeit, Netze minimal-invasiv in unterschiedliche Bauchdeckenschichten zu implantieren und gleichzeitig eine morphologische und funktionelle Rekonstruktion der Bauchdecke zu erreichen. In diesem Videobeitrag wird die Versorgung von Ventral- und Inzisionalhernien mit Netzimplantation in den präperitonealen Raum (robotische ventrale transabdominelle präperitoneale Patchplastik, rv-TAPP) sowie in den retrorektalen Raum (r-Rives bzw. robotische transabdominelle retromuskuläre umbilikale Patchplastik [r-TARUP]) präsentiert. Es werden die Ergebnisse einer Kohortenstudie an 118 konsekutiven Patienten vorgestellt und im Hinblick auf den Mehrwert der robotischen Technik in der Extraperitonealisierung der Netze und in der Weiterbildung diskutiert.
Das Prinzip der gezielten Trennung bzw. Schwächung einzelner Komponenten der Bauchdecke zur Spannungsentlastung der Medianlinie bei großen abdominellen Rekonstruktionen ist seit über 30 Jahren als anteriore Komponentenseparation (aKS) bekannt und ein etabliertes Verfahren. Auf der Suche nach Alternativen mit geringerer Komplikationsrate wurde die posteriore Komponentenseparation (pKS) entwickelt; der „transversus abdominis release“ (TAR) ist eine nervenschonende Modifikation der pKS. Mit den ergonomischen Ressourcen der Robotik (z. B. abgewinkelte Instrumente) kann der TAR minimal-invasiv durchgeführt werden (r-TAR): Bruchlücken von bis zu 14 cm lassen sich verschließen und ein großes extraperitoneales Netz implantieren. In diesem Videobeitrag wird die Versorgung großer Inzisionalhernien in der r‑TAR-Technik präsentiert. Exemplarisch werden die Ergebnisse einer Kohortenstudie an 13 konsekutiven Patienten vorgestellt. Der Eingriff ist anspruchsvoll, die eigenen Ergebnisse sind – wie auch die Berichte aus der Literatur – ermutigend. Der r‑TAR entwickelt sich zur Königsdisziplin der Bauchdeckenrekonstruktion.
The use of human adipose-derived mesenchymal stem cells (ASCs) for cell-based therapeutic approaches, in terms of repair and regeneration of various tissues and organs, offers an alternative therapeutic tool in the field of regenerative medicine. The ability of ASCs to differentiate along mesenchymal lineages is not the only property that makes these cells particularly attractive for therapeutic purposes. Their promising functions in promoting angiogenesis, reducing inflammation as well as in functional tissue restoration are largely related to the trophic effects of a broad panel of secreted cytokines and growth factors. However, in cell-based approaches, the cell-loaded construct often is exposed to an ischemic microenvironment characterized by severe oxidative and nutritional stress after transplantation due to the initial lack of vascular connection, resulting in reduced cell viability and altered cell behaviour. Therefore, the effective use of ASCs in regenerative medicine first requires a comprehensive characterization of the cells in terms of their viability, differentiation capacity and especially their secretory capabilities under ischemia-mimicking conditions in order to better understand their beneficial role. Accordingly, in the first part of this work, ASCs were investigated under different ischemic conditions, in which cells were exposed to both glucose and oxygen deprivation, with respect to viability and secretory function. Using mRNA gene expression analysis, significantly higher expression of selected angiogenic, anti-apoptotic and immunomodulatory factors (IL-6, VEGF, STC-1) could be demonstrated under harsh ischemic conditions. These results were reflected at the protein expression level by a significantly increased secretion of these factors. For stanniocalcin-1 (STC-1), a factor not yet described in ASCs, a particularly high expression with significant secreted amounts of the protein could be demonstrated under harsh ischemic conditions. Thus, the first part of this work, in addition to the characterization of the viability, provided first insights into the secretory response of ASCs under ischemic conditions.
The response of ASCs to glucose deficiency in combination with severe hypoxia has been little explored to date. Thus, the focus of the second part of this work was on a more detailed investigation of the secretory response of ASCs under glucose and oxygen deprivation. For a more comprehensive analysis of the secretion profile, a cytokine antibody array was performed, which allowed the detection of a broad panel of secreted angiogenic factors
(IL-8, ANG), matrix-regulating proteins (TIMP-1, TIMP-2), chemokines (MCP-1/CCL2,
IP-10/CXCL 10) and other factors under ischemic conditions. To verify these results, selected factors were examined using ELISA. The analysis revealed that the secretion of individual factors (e.g., STC-1, VEGF) was significantly upregulated by the combination of glucose and oxygen deprivation compared to oxygen deprivation alone.
In order to investigate the impact of the secretome of ischemic ASCs on cell types involved in tissue regeneration, the effect of conditioned medium of ischemia-challenged ASCs on both endothelial cells and fibroblasts was investigated in subsequent experiments. Significantly increased viability and tube formation of endothelial cells as well as activated migration of fibroblasts by the secreted factors of ischemic ASCs could be demonstrated. A direct correlation of these effects to STC-1, which was significantly upregulated under ischemic conditions and has been described as a regulator of key cellular functions, could not be verified.
The particular secretory capacity of ASCs provides a valuable tool for cell-based therapies, such as cell-assisted lipotransfer (CAL), where by enriching fat grafts with isolated ASCs, a significantly improved survival rate of the transplanted construct is achieved with less resorption of the fat tissue as well as a reduction in adverse implications, such as fibrosis and cyst formation. In order to better understand the function of ASCs in CAL, an autologous transwell-based lipograft-ASC co-culture was established in the last part of this work, in which first investigations showed a markedly increased secretion of VEGF compared to lipografts without added ASCs. As the stability rate of the fat tissue and thus the success of CAL is presumably also dependent on the preparation of the tissue before transplantation, the conventional preparation method of fat tissue for vocal fold augmentation in laryngoplasty was additionally evaluated in vitro in a pilot experiment. By analyzing the viability and tissue structure of the clinically prepared injection material, a large number of dead cells and a clearly damaged tissue structure with necrotic areas could be demonstrated. In comparison, the preparation method of the fat tissue established in this work as small tissue fragments was able to provide a clearly intact, vital, and vascularized tissue structure. This type of adipose tissue preparation represents a promising alternative for clinical vocal fold augmentation.
In conclusion, the results of this work contribute to a comprehensive characterization of ASCs under ischemic conditions, such as those prevalent at the transplantation site or in tissue regeneration. The results obtained, especially on the secretory capacity of ASCs, provide new insights into how ASCs mediate regenerative effects in an ischemic milieu and why their use for therapeutic purposes is highly attractive and promising.
Cell death is an essential aspect of life that plays an important role for successful development and tissue remodeling as well as for diseases. There are several different types of cell death that differ from each other in morphological, functional and biochemical ways. Regulated cell death that occurs in physiological processes is generally equated with programmed cell death (PCD), whereby apoptosis is the most studied form of PCD. Ferroptosis is a form of regulated cell death and unique in its requirements for iron and lipid peroxidation. It is linked to numerous biological processes, such as amino acid metabolism, phospholipid metabolism and sterol synthesis. Cholesterol biosynthesis is a complex pathway with a large number of enzymes and substrates that are potential target points for cellular dysfunctions. Motivated by the results from a CRISPR-based genetic screening in this thesis, we focused on 7-dehydrocholesterol reductase (DHCR7), the enzyme responsible for conversion of 7-dehydrocholesterol (7-DHC) to cholesterol. In this work we focused on the ferroptosis sensitive cell line HT1080 and generated a series of models to address the importance of DHCR7 in ferroptosis. Using CRISPR/Cas9, HT1080 DHCR7_KO and DHCR7/SC5D_KO cell lines were generated and used to validate their sensitivity against ferroptosis inducers and sterol consumption. We could show that 7-DHC is a strong antiferroptotic agent that could prevent cell death in genetic models as well as when supplemented directly to cells. Importantly, all the results obtained were subsequently confirmed in isogenic reconstituted pairs from the HT1080 DHCR7/SC5D_KO. Moreover, we demonstrate that this protective effect is not due to an inherent and unspecific resistance as the sensitivity to non-ferroptotic stimuli was equally effective in killing the HT1080 DHCR7_KO and DHCR7/SC5D_KO cell lines. We could also show that selenium present in the media has a strong impact on the activity of 7-DHC and this is because in its absence the effective concentration is rapidly decreased. Surprisingly we also demonstrate that removing sterol from cell culture triggers ferroptosis in cells unable to synthesize 7-DHC, suggestive that this could be used as a novel mechanism to trigger ferroptosis. Ultimately, in the present work we could show that unlike previously reported, 7-DHC is not only a toxic intermediate of the cholesterol biosynthesis pathway but under specific circumstances it has a strong pro-survival effect.
In peripheral nervous system (PNS), the blood-nerve barrier (BNB) and myelin barrier (MB) are important physiological fences for maintaining the environment for axons, Schwann cells and other associated cells within peripheral nerves. The perineurium surrounding the nerves and endoneurial vessels nourishing the nerves compose the BNB. Schwann cells wrapping around neurons form the MB. Destruction or malfunction of the barriers has been postulated as an initial step in the development of pathologic conditions concerning human peripheral nerves, such as traumatic neuropathy and the disease of chronic inflammatory demyelination polyneuropathy (CIDP).
Tight junction proteins (TJPs) are intercellular junctions building the microstructure of barriers. They play a key role in tightly connecting adjacent cells, controlling the passage of ions, water and other molecules via the paracellular pathway, and maintaining the cell polarity. Among the family of TJPs, claudins are the major structural components which form the backbone of TJs. Certain key TJPs [e.g. claudins (claudin-1, -5, -19, occludin, zona occludens (ZO-1)] have been identified in neural barriers and explored for therapeutic targets. The expression of Cldn12 gene has been documented in human/rodent tibial nerves, spinal cord and DRG. However, the role of claudin-12 in PNS is unknown.
In the present study, we firstly found a loss of claudin-12 immunoreactivity (IR) in male or postmenopausal female patients with painful CIDP or non-inflammatory polyneuropathy (PNP). Then, we utilized male and female Cldn12-KO mice and the chronic constriction injury (CCI) model. Cldn12 mRNA and IR were reduced in WT mice after nerve injury. Deletion of Cldn12 via general knockout (KO) induced mechanical allodynia at baseline level and after CCI in time-dependent manner in male mice. KO of Cldn12 in males resulted in loss of small axons, perineurial barrier and MB breakdown, as well as TJP complex disruption with claudin-1, -19 and Pmp22 reduction. Moreover, local Cldn12 siRNA application mimicked mechanical allodynia and MB breakdown.
In conclusion, claudin-12 deficiency is associated with painful CIDP/non-inflammatory PNP. Claudin-12 is a regulatory TJP crucial for mechanical nociception, perineurial barrier and MB integrity, and proper TJP composition in mice. Therefore, further investigating the functions of claudin-12 and its mechanism is important to prompt the development of new therapeutic approaches for painful neuropathies.
Adrenocortical carcinoma (ACC) is a rare endocrine malignancy and treatment of advanced disease is challenging. Clinical trials with multi-tyrosine kinase inhibitors in the past have yielded disappointing results. Here, we investigated fibroblast growth factor (FGF) receptors and their pathways in adrenocortical tumors as potential treatment targets. We performed real-time RT-PCR of 93 FGF pathway related genes in a cohort of 39 fresh frozen benign and malignant adrenocortical, 9 non-adrenal tissues and 4 cell lines. The expression of FGF receptors was validated in 166 formalin-fixed paraffin embedded (FFPE) tissues using RNA in situ hybridization (RNAscope) and correlated with clinical data. In malignant compared to benign adrenal tumors, we found significant differences in the expression of 16/94 FGF receptor pathway related genes. Genes involved in tissue differentiation and metastatic spread through epithelial to mesechymal transition were most strongly altered. The therapeutically targetable FGF receptors 1 and 4 were upregulated 4.6- and 6-fold, respectively, in malignant compared to benign adrenocortical tumors, which was confirmed by RNAscope in FFPE samples. High expression of FGFR1 and 4 was significantly associated with worse patient prognosis in univariate analysis. After multivariate adjustment for the known prognostic factors Ki-67 and ENSAT tumor stage, FGFR1 remained significantly associated with recurrence-free survival (HR=6.10, 95%CI: 1.78 – 20.86, p=0.004) and FGFR4 with overall survival (HR=3.23, 95%CI: 1.52 – 6.88, p=0.002). Collectively, our study supports a role of FGF pathways in malignant adrenocortical tumors. Quantification of FGF receptors may enable a stratification of ACC for the use of FGFR inhibitors in future clinical trials.
Chlamydia trachomatis, an obligate intracellular human pathogen, is the world’s leading cause of infection related blindness and the most common, bacterial sexually transmitted disease. In order to establish an optimal replicative niche, the pathogen extensively interferes with the physiology of the host cell. Chlamydia switches in its complex developmental cycle between the infectious non-replicative elementary bodies (EBs) and the non-infectious replicative reticulate bodies (RBs). The transformation to RBs, shortly after entering a host cell, is a crucial process in infection to start chlamydial replication. Currently it is unknown how the transition from EBs to RBs is initiated. In this thesis, we could show that, in an axenic media approach, L glutamine uptake by the pathogen is crucial to initiate the EB to RB transition. L-glutamine is converted to amino acids which are used by the bacteria to synthesize peptidoglycan. Peptidoglycan inturn is believed to function in separating dividing Chlamydia. The glutamine metabolism is reprogrammed in infected cells in a c-Myc-dependent manner, in order to accomplish the increased requirement for L-glutamine. Upon a chlamydial infection, the proto-oncogene c-Myc gets upregulated to promote host cell glutaminolysis via glutaminase GLS1 and the L-glutamine transporter SLC1A5/ASCT2. Interference with this metabolic reprogramming leads to limited growth of C. trachomatis. Besides the active infection, Chlamydia can persist over a long period of time within the host cell whereby chronic and recurrent infections establish. C. trachomatis acquire a persistent state during an immune attack in response to elevated interferon-γ (IFN-γ) levels. It has been shown that IFN-γ activates the catabolic depletion of L-tryptophan via indoleamine 2,3-dioxygenase (IDO), resulting in the formation of non-infectious atypical chlamydial forms. In this thesis, we could show that IFN-γ depletes the key metabolic regulator c-Myc, which has been demonstrated to be a prerequisite for chlamydial development and growth, in a STAT1-dependent manner. Moreover, metabolic analyses revealed that the pathogen de routs the host cell TCA cycle to enrich pyrimidine biosynthesis. Supplementing pyrimidines or a-ketoglutarate helps the bacteria to partially overcome the persistent state. Together, the results indicate a central role of c-Myc induced host glutamine metabolism reprogramming and L-glutamine for the development of C. trachomatis, which may provide a basis for anti-infectious strategies. Furthermore, they challenge the longstanding hypothesis of L-tryptophan shortage as the sole reason for IFN-γ induced persistence and suggest a pivotal role of c-Myc in the control of the C. trachomatis dormancy.
Background: The hypothalamus is an important brain region for the regulation of energy balance. Roux-en-Y gastric bypass (RYGB) surgery and gut hormone-based treatments are known to reduce body weight, but their effects on hypothalamic gene expression and signaling pathways are poorly studied. Methods: Diet-induced obese male Wistar rats were randomized into the following groups: RYGB, sham operation, sham + body weight-matched (BWM) to the RYGB group, osmotic minipump delivering PYY3-36 (0.1 mg/kg/day), liraglutide s.c. (0.4 mg/kg/day), PYY3-36 + liraglutide, and saline. All groups (except BWM) were kept on a free choice of high- and low-fat diets. Four weeks after interventions, hypothalami were collected for RNA sequencing. Results: While rats in the RYGB, BWM, and PYY3-36 + liraglutide groups had comparable reductions in body weight, only RYGB and BWM treatment had a major impact on hypothalamic gene expression. In these groups, hypothalamic leptin receptor expression as well as the JAK–STAT, PI3K-Akt, and AMPK signaling pathways were upregulated. No significant changes could be detected in PYY3-36 + liraglutide-, liraglutide-, and PYY-treated groups. Conclusions: Despite causing similar body weight changes compared to RYGB and BWM, PYY3-36 + liraglutide treatment does not impact hypothalamic gene expression. Whether this striking difference is favorable or unfavorable to metabolic health in the long term requires further investigation.
Purpose
Knowledge on Ruxolitinib exposure in patients with graft versus host disease (GvHD) is scarce. The purpose of this prospective study was to analyze Ruxolitinib concentrations of GvHD patients and to investigate effects of CYP3A4 and CYP2C9 inhibitors and other covariates as well as concentration-dependent effects.
Methods
262 blood samples of 29 patients with acute or chronic GvHD who were administered Ruxolitinib during clinical routine were analyzed. A population pharmacokinetic model obtained from myelofibrosis patients was adapted to our population and was used to identify relevant pharmacokinetic properties and covariates on drug exposure. Relationships between Ruxolitinib exposure and adverse events were assessed.
Results
Median of individual mean trough serum concentrations was 39.9 ng/mL at 10 mg twice daily (IQR 27.1 ng/mL, range 5.6-99.8 ng/mL). Applying a population pharmacokinetic model revealed that concentrations in our cohort were significantly higher compared to myelofibrosis patients receiving the same daily dose (p < 0.001). Increased Ruxolitinib exposure was caused by a significant reduction in Ruxolitinib clearance by approximately 50%. Additional comedication with at least one strong CYP3A4 or CYP2C9 inhibitor led to a further reduction by 15% (p < 0.05). No other covariate affected pharmacokinetics significantly. Mean trough concentrations of patients requiring dose reduction related to adverse events were significantly elevated (p < 0.05).
Conclusion
Ruxolitinib exposure is increased in GvHD patients in comparison to myelofibrosis patients due to reduced clearance and comedication with CYP3A4 or CYP2C9 inhibitors. Elevated Ruxolitinib trough concentrations might be a surrogate for toxicity.
Die Arbeitsgruppe Informationskompetenz im Bibliotheksverbund Bayern (AGIK Bay) veröffentlichte im Oktober 2020 das interaktive E-Tutorial S.P.U.T.N.I.K. zur Förderung der Informationskompetenz von Schüler*innen der Oberstufe, das sich inhaltlich am Framework for Information Literacy for Higher Education orientiert. Der Artikel zeichnet die Entstehung des Tutorials in Kooperation mit Studierenden des Fachbereichs Archiv- und Bibliothekswesen der Hochschule für den öffentlichen Dienst in Bayern nach und skizziert die Herausforderungen bei der finalen Umsetzung durch die Universitätsbibliothek Würzburg. Das Tutorial ist als Moodle- und Ilias-kompatibles SCORM-Lernpaket mit vielen interaktiven Quizelementen konzipiert und in fünf Modulen aufgebaut. Es ist online zugänglich und steht zur Nachnutzung innerhalb der DACH-Bibliothekscommunity unter einer Creative Commons Lizenz bereit.
Background: COVID-19 patients are at high thrombotic risk. The safety and efficacy of different anticoagulation regimens in COVID-19 patients remain unclear. Methods: We searched for randomised controlled trials (RCTs) comparing intermediate- or therapeutic-dose anticoagulation to standard thromboprophylaxis in hospitalised patients with COVID-19 irrespective of disease severity. To assess efficacy and safety, we meta-analysed data for all-cause mortality, clinical status, thrombotic event or death, and major bleedings. Results: Eight RCTs, including 5580 patients, were identified, with two comparing intermediate- and six therapeutic-dose anticoagulation to standard thromboprophylaxis. Intermediate-dose anticoagulation may have little or no effect on any thrombotic event or death (RR 1.03, 95% CI 0.86–1.24), but may increase major bleedings (RR 1.48, 95% CI 0.53–4.15) in moderate to severe COVID-19 patients. Therapeutic-dose anticoagulation may decrease any thrombotic event or death in patients with moderate COVID-19 (RR 0.64, 95% CI 0.38–1.07), but may have little or no effect in patients with severe disease (RR 0.98, 95% CI 0.86–1.12). The risk of major bleedings may increase independent of disease severity (RR 1.78, 95% CI 1.15–2.74). Conclusions: Certainty of evidence is still low. Moderately affected COVID-19 patients may benefit from therapeutic-dose anticoagulation, but the risk for bleeding is increased.
Background
Acute respiratory distress syndrome (ARDS) is a complex clinical diagnosis with various possible etiologies. One common feature, however, is pulmonary permeability edema, which leads to an increased alveolar diffusion pathway and, subsequently, impaired oxygenation and decarboxylation. A novel inhaled peptide agent (AP301, solnatide) was shown to markedly reduce pulmonary edema in animal models of ARDS and to be safe to administer to healthy humans in a Phase I clinical trial. Here, we present the protocol for a Phase IIB clinical trial investigating the safety and possible future efficacy endpoints in ARDS patients.
Methods
This is a randomized, placebo-controlled, double-blind intervention study. Patients with moderate to severe ARDS in need of mechanical ventilation will be randomized to parallel groups receiving escalating doses of solnatide or placebo, respectively. Before advancing to a higher dose, a data safety monitoring board will investigate the data from previous patients for any indication of patient safety violations. The intervention (application of the investigational drug) takes places twice daily over the course of 7 days, ensued by a follow-up period of another 21 days.
Discussion
The patients to be included in this trial will be severely sick and in need of mechanical ventilation. The amount of data to be collected upon screening and during the course of the intervention phase is substantial and the potential timeframe for inclusion of any given patient is short. However, when prepared properly, adherence to this protocol will make for the acquisition of reliable data. Particular diligence needs to be exercised with respect to informed consent, because eligible patients will most likely be comatose and/or deeply sedated at the time of inclusion.

Background
Up to 20 per cent of all operations for patients with colorectal cancer (CRC) are performed in octogenarians. Anastomotic leakage is a leading cause of morbidity and death after resection for CRC. The aim of this study was to assess the rate of anastomosis creation, the risk of anastomotic leakage and death in surgery for left-sided CRC in elderly patients.
Methods
This prospective cohort study compared patients less than 80 and 80 or more years with left-sided CRC resection performed between 2013 and 2019. Data were provided from a risk-adjusted surgical quality-assessment system with 219 participating centres in Germany. Outcome measures were the rate of anastomoses, anastomotic leakages, death at 30 days and 2-year overall survival (OS). Propensity score matching was used to control for selection bias and compare subgroups of patients of less than 80 and 80 or more years.
Results
Out of 18 959 patients, some 3169 (16.7 per cent) were octogenarians. Octogenarians were less likely to receive anastomoses (82.0 versus 92.9 per cent, P < 0.001; odds ratio 0.50 (95 per cent c.i. 0.44 to 0.58), P < 0.001). The rate of anastomotic leakages did not differ between age groups (8.6 versus 9.7 per cent, P = 0.084), but 30-day mortality rate after leakage was significantly higher in octogenarians (15.8 versus 3.5 per cent, P < 0.001). Overall, anastomotic leakage was the strongest predictor for death (odds ratio 4.95 (95 per cent c.i. 3.66 to 6.66), P < 0.001). In the subgroup with no leakage, octogenarians had a lower 2-year OS rate than younger patients (71 versus 87 per cent, P < 0.001), and in the population with anastomotic leakage, the 2-year OS was 80 per cent in younger and 43 per cent in elderly patients (P < 0.001). After propensity score matching, older age remained predictive for not receiving an anastomosis (odds ratio 0.54 (95 per cent c.i. 0.46 to 0.63), P < 0.001) and for death (odds ratio 2.60 (95 per cent c.i. 1.78 to 3.84), P < 0.001), but not for the occurrence of leakages (odds ratio 0.94 (95 per cent c.i. 0.76 to 1.15), P = 0.524).
Conclusion
Anastomotic leakage is not more common in octogenarians, but an age of 80 years or older is an independent factor for not receiving an anastomosis in surgery for left-sided CRC. The mortality rate in the case of leakage in octogenarians was reported to exceed 15 per cent.
We performed a systematic search and meta-analysis of available literature to determine the safety profile of Cerebrolysin in acute ischemic stroke, filling existing safety information gaps and inconsistent results. We searched EMBASE, PubMed, and Cochrane Databases of Systematic Reviews and Clinical Trials up to the end of February 2021. Data collection and analysis were conducted using methods described in the Cochrane Handbook for Systematic Reviews of Interventions. All safety outcomes were analyzed based on risk ratios (RR) and their 95% confidence intervals. The meta-analysis pooled 2202 patients from twelve randomized clinical trials, registering non-statistically significant (p > 0.05) differences between Cerebrolysin and placebo throughout main and subgroup analyses. The lowest rate of Serious Adverse Events (SAE), as compared to placebo, was observed for the highest dose of Cerebrolysin (50 mL), highlighting a moderate reduction (RR = 0.6). We observed a tendency of superiority of Cerebrolysin regarding SAE in high dose treatment courses for moderate-severe ischemic stroke, suggesting some effect of the agent against adverse events. This comprehensive safety meta-analysis confirms the safety profile for patients treated with Cerebrolysin after acute ischemic stroke, as compared to placebo.
Background
Patients with coronavirus disease 2019 (COVID-19) who undergo surgery have impaired postoperative outcomes and increased mortality. Consequently, elective and semi-urgent operations on the increasing number of patients severely affected by COVID-19 have been indefinitely postponed.in many countries with unclear implications on disease progression and overall survival. The purpose of this study was to evaluate whether the establishment of a standardized screening program for acute respiratory syndrome coronavirus 2 (SARS-CoV-2) is sufficient to ensure high-quality medical and surgical treatment of COVID-19 and non-COVID-19 patients while minimizing in-hospital SARS-CoV-2 transmission.
Methods
The screening program comprised polymerase chain reaction (PCR) testing of nasopharyngeal swabs and a standardized questionnaire about potential symptoms for SARS-CoV-2 infection. All elective and emergency patients admitted to the surgical department of a tertiary-care hospital center in Lower Franconia, Germany, between March and May 2020 were included and their characteristics were recorded.
Results
Out of the study population (n = 657), 509 patients (77.5%) had at least one risk factor for a potentially severe course of COVID-19 and 164 patients (25%) were active smokers. The average 7-day incidence in Lower Franconia was 24.0/100,000 during the observation period. Preoperative PCR testing revealed four asymptomatic positive patients out of the 657 tested patients. No postoperative SARS-CoV-2 infection or transmission could be detected.
Conclusion
The implementation of a standardized preoperative screening program to both COVID-19 and non-COVID-19 patients can ensure high-quality surgical care while minimizing infection risk for healthcare workers and potential in-hospital transmission.
Epidermal fragments enriched in guard cells (GCs) were isolated from the halophyte quinoa (Chenopodium quinoa Wild.) species, and the response at the proteome level was studied after salinity treatment of 300 mM NaCl for 3 weeks. In total, 2147 proteins were identified, of which 36% were differentially expressed in response to salinity stress in GCs. Up and downregulated proteins included signaling molecules, enzyme modulators, transcription factors and oxidoreductases. The most abundant proteins induced by salt treatment were desiccation-responsive protein 29B (50-fold), osmotin-like protein OSML13 (13-fold), polycystin-1, lipoxygenase, alpha-toxin, and triacylglycerol lipase (PLAT) domain-containing protein 3-like (eight-fold), and dehydrin early responsive to dehydration (ERD14) (eight-fold). Ten proteins related to the gene ontology term “response to ABA” were upregulated in quinoa GC; this included aspartic protease, phospholipase D and plastid-lipid-associated protein. Additionally, seven proteins in the sucrose–starch pathway were upregulated in the GC in response to salinity stress, and accumulation of tryptophan synthase and L-methionine synthase (enzymes involved in the amino acid biosynthesis) was observed. Exogenous application of sucrose and tryptophan, L-methionine resulted in reduction in stomatal aperture and conductance, which could be advantageous for plants under salt stress. Eight aspartic proteinase proteins were highly upregulated in GCs of quinoa, and exogenous application of pepstatin A (an inhibitor of aspartic proteinase) was accompanied by higher oxidative stress and extremely low stomatal aperture and conductance, suggesting a possible role of aspartic proteinase in mitigating oxidative stress induced by saline conditions.
Sarcopenia and Malnutrition Screening in Female Osteoporosis Patients — A Cross-Sectional Study

Sarcopenia and malnutrition are important determinants of increased fracture risk in osteoporosis. SARC-F and MNA-SF are well-established questionnaires for identifying patients at risk for these conditions. We sought to evaluate the feasibility and potential added benefit of such assessments as well as the actual prevalence of these conditions in osteoporosis patients. We conducted a cross-sectional, single-center study in female osteoporosis patients ≥ 65 years (SaNSiBaR-study). Results of the sarcopenia (SARC-F) and malnutrition (MNA-SF) screening questionnaires were matched with a functional assessment for sarcopenia and data from patients’ medical records. Out of 107 patients included in the analysis, a risk for sarcopenia (SARC-F ≥ 4 points) and a risk for malnutrition (MNA-SF ≤ 11 points) was found in 33 (30.8%) and 38 (35.5%) patients, respectively. Diagnostic overlap with coincident indicative findings in both questionnaires was observed in 17 patients (16%). As compared to the respective not-at-risk groups, the mean short physical performance battery (SPPB) score was significantly reduced in both patients at risk for sarcopenia (7.0 vs. 10.9 points, p < 0.001) and patients at risk for malnutrition (8.7 vs. 10.5 points, p = 0.005). Still, confirmed sarcopenia according to EWGSOP2 criteria was present in only 6 (6%) of all 107 patients, with only 3 of them having an indicative SARC-F score. Bone mineral density was not significantly different in any of the at-risk groups at any site. In summary, applying SARC-F and MNA-SF in osteoporosis patients appears to be a complementary approach to identify individuals with functional deficits.
SARS-CoV-2 infection can cause fatal inflammatory lung pathology, including thrombosis and increased pulmonary vascular permeability leading to edema and hemorrhage. In addition to the lung, cytokine storm-induced inflammatory cascade also affects other organs. SARS-CoV-2 infection-related vascular inflammation is characterized by endotheliopathy in the lung and other organs. Whether SARS-CoV-2 causes endotheliopathy by directly infecting endothelial cells is not known and is the focus of the present study. We observed 1) the co-localization of SARS-CoV-2 with the endothelial cell marker CD31 in the lungs of SARS-CoV-2-infected mice expressing hACE2 in the lung by intranasal delivery of adenovirus 5-hACE2 (Ad5-hACE2 mice) and non-human primates at both the protein and RNA levels, and 2) SARS-CoV-2 proteins in endothelial cells by immunogold labeling and electron microscopic analysis. We also detected the co-localization of SARS-CoV-2 with CD31 in autopsied lung tissue obtained from patients who died from severe COVID-19. Comparative analysis of RNA sequencing data of the lungs of infected Ad5-hACE2 and Ad5-empty (control) mice revealed upregulated KRAS signaling pathway, a well-known pathway for cellular activation and dysfunction. Further, we showed that SARS-CoV-2 directly infects mature mouse aortic endothelial cells (AoECs) that were activated by performing an aortic sprouting assay prior to exposure to SARS-CoV-2. This was demonstrated by co-localization of SARS-CoV-2 and CD34 by immunostaining and detection of viral particles in electron microscopic studies. Moreover, the activated AoECs became positive for ACE-2 but not quiescent AoECs. Together, our results indicate that in addition to pneumocytes, SARS-CoV-2 also directly infects mature vascular endothelial cells in vivo and ex vivo, which may contribute to cardiovascular complications in SARS-CoV-2 infection, including multipleorgan failure.
Clinical trials of novel therapeutics for Alzheimer’s Disease (AD) have consumed a significant amount of time and resources with largely negative results. Repurposing drugs already approved by the Food and Drug Administration (FDA), European Medicines Agency (EMA), or Worldwide for another indication is a more rapid and less expensive option. Therefore, we apply the scaffold searching approach based on known amyloid-beta (Aβ) inhibitor tramiprosate to screen the DrugCentral database (n = 4,642) of clinically tested drugs. As a result, menadione bisulfite and camphotamide substances with protrombogenic and neurostimulation/cardioprotection effects were identified as promising Aβ inhibitors with an improved binding affinity (ΔGbind) and blood-brain barrier permeation (logBB). Finally, the data was also confirmed by molecular dynamics simulations using implicit solvation, in particular as Molecular Mechanics Generalized Born Surface Area (MM-GBSA) model. Overall, the proposed in silico pipeline can be implemented through the early stage rational drug design to nominate some lead candidates for AD, which will be further validated in vitro and in vivo, and, finally, in a clinical trial.
