Anästhetika-induzierte Präkonditionierung (APC) mit Desfluran vermittelt einen endogenen Schutzmechanismus gegen den Ischämie/Reperfusionsschaden im Tiermodell mit männlichen C57BL/6 Mäusen. Die Calcium-Calmodulinkinase IIδ (CamKIIδ) spielt eine zentrale Rolle im intrazellulären Calciumhaushalt und beim Ischämie/Reperfusionsschaden.
Ein Ergebnis dieser Studie zeigt, dass eine Desfluran-vermittelte Reduktion der Herzinfarktgröße in Wildtyptieren, nicht aber in Mäusen mit homozygotem genetischen Knock-Out (KO) der CaMKIIδ stattfindet. Weiterhin wird gezeigt, dass der CaMKIIδ KO die Expression von Phospholamban (PLB) und die Phosphorylierung von PLB an der Aminosäurestelle Serin 16 im Vergleich zum Wildtyp mehr als verdoppelt, ermittelt via Western-Immunoblotting. Darüber hinaus wird gezeigt, dass die Expression des β1 –Adrenorezeptors (AR) in Tieren mit homo- und heterozygotem CaMKIIδ KO im Vergleich zum Wildtyp signifikant erhöht ist. APC mit Desfluran hebt diese erhöhte β1 –AR Konzentrationen im homozygoten, aber nicht im heterozygoten KO auf. Dies zeigt, dass sich die β1 –AR Expression entsprechend der CaMKIIδ Verfügbarkeit adaptiert, dieser Effekt aber durch APC unterbunden werden kann.
Aging is a one-way process associated with profound structural and functional changes in the organism. Indeed, the neuromuscular system undergoes a wide remodeling, which involves muscles, fascia, and the central and peripheral nervous systems. As a result, intrinsic features of tissues, as well as their functional and structural coupling, are affected and a decline in overall physical performance occurs. Evidence from the scientific literature demonstrates that senescence is associated with increased stiffness and reduced elasticity of fascia, as well as loss of skeletal muscle mass, strength, and regenerative potential. The interaction between muscular and fascial structures is also weakened. As for the nervous system, aging leads to motor cortex atrophy, reduced motor cortical excitability, and plasticity, thus leading to accumulation of denervated muscle fibers. As a result, the magnitude of force generated by the neuromuscular apparatus, its transmission along the myofascial chain, joint mobility, and movement coordination are impaired. In this review, we summarize the evidence about the deleterious effect of aging on skeletal muscle, fascial tissue, and the nervous system. In particular, we address the structural and functional changes occurring within and between these tissues and discuss the effect of inflammation in aging. From the clinical perspective, this article outlines promising approaches for analyzing the composition and the viscoelastic properties of skeletal muscle, such as ultrasonography and elastography, which could be applied for a better understanding of musculoskeletal modifications occurring with aging. Moreover, we describe the use of tissue manipulation techniques, such as massage, traction, mobilization as well as acupuncture, dry needling, and nerve block, to enhance fascial repair.
Es wurden in einer retrospektiven Analyse die Einsatzprotokolle von 1820 First Responder-Einsätzen aus dem Bereich Stadt und Landkreis Würzburg des Jahres 2001 ausgewertet. Zum Vergleich wurden die Einsatzzeiten aus Leitstellenprotokollen von 2663 Notfalleinsätzen herangezogen. Die First Responder wurden bei einer 24– Stunden–Verfügbarkeit die ganze Woche über gleich häufig, den Wochenenden allerdings zu 5% mehr Einsätzen alarmiert. Es zeigte sich jedoch, dass auch die Häufigkeit der Notarzteinsätze am Wochenende um 3% höher war als unter der Woche und die höhere Einsatzanzahl nicht mit einer besseren Verfügbarkeit der First Responder am Wochenende zu erklären ist. Ähnlich zeigte sich die Situation bei der Verteilung der Einsätze über den Tag, bei der die meisten First Responder–Einsätze pro Stunde im Zeitraum zwischen 8 und 23 Uhr absolviert werden in Übereinstimmung mit der Anzahl der Notarzteinsätze. Das Meldebild der Leitstelle hatte keinen Einfluss auf das Eintreffen am Notfallort. Im Gegensatz dazu erreichten das NEF und der RTW nicht einmal 30% der Einsatzorte innerhalb von 7 Minuten ( 21,3%, bzw. 23,5%). Der RTH erreichte immerhin 45 % der Patienten in dieser Zeit. Durch die First Responder konnte somit eine deutliche Verkürzung des therapiefreien Intervalls erreicht werden. Die First Responder im Leitstellenbereich Würzburg hatten bei 44,8% der Einsätze einen Zeitvorteil von durchschnittlich 5,03±3,7 Minuten. Selbst First Responder–Gruppen, die in der Nähe einer Rettungswache stationiert sind, konnten in vielen Fällen einen signifikanten Zeitvorteil von 3-4 Minuten gegenüber dem Rettungsdienst erreichen. Der Zeitvorteil war in ländlichen Regionen signifikant größer als in städtischen. D.h. der Standort eines First–Responder–Systems kann nicht rein mathematisch mit einem bestimmten Radius um eine Rettungswache gezogen werden. Betrachtet man den Ausbildungskatalog der First Responder und die angetroffenen Notfälle, so scheint die Ausbildung der First Responder für die erforderliche Hilfeleistung zur Überbrückung des therapiefreien Intervalls ausreichend zu sein. 8,2% der Patienten waren bewusstlos, 14,3% hatten Atemnot, bei 4,8% lag ein isolierter Atemstillstand vor. In 12% lag ein Schock und bei 4,4% ein Kreislaufstillstand vor. 67% der ergriffenen Hilfsmaßnahmen waren allgemeiner Natur, während in 27,1% 63 spezifische notfallmedizinische Maßnahmen durchgeführt wurden. Der Anteil der Reanimationen am Gesamteinsatzaufkommen betrug 3%. Der Anteil der primär erfolgreichen Reanimationen lag bei 15,8%. Auffällig dabei war, dass es sich bei den erfolgreichen Reanimationen in allen Fällen um Kreislaufstillstände aus internistischer Ursache handelte.Der Einsatzort wurde bei den erfolgreichen Reanimationen im Durchschnitt eine Minute später erreicht, als bei den nicht erfolgreichen, wobei 77% der Einsatzorte bei erfolgreicher Reanimation innerhalb von 5 Minuten erreicht wurden. Innerhalb von 4-7 Minuten konnten jedoch nur 17,7% der NEFs und 20,5% der RTWs den Einsatzort erreichen, so dass durch den Einsatz der First Responder eine Steigerung der Reanimationschancen erreicht werden konnte. Die Effektivität des Zeitvorteils könnte durch die Ausstattung der First Responder mit AEDs um bis zu 22% gesteigert werden, da die Frühdefibrillation neben der cardiopulmonalen Reanimation das einzig probate Mittel zur Steigerung der Überlebensrate bei Kammerflimmern ist. Es sollte die Ausbildung in Frühdefibrillation allerdings nicht mehr fakultativ, sondern verpflichtend sein. Der Einsatz von First Respondern stellt damit ein wirksames Konzept zur Ergänzung des professionellen Rettungsdienstes dar und sollte deshalb weiter intensiviert werden.
Der Einsatz von Regionalanästhesien nahm in den letzten Jahren stark zu. Durch die Blockade von spannungsabhängigen Natriumkanälen verhindern Lokalanästhetika eine Depolarisation sowie die neuronale Fortleitung der Schmerzimpulse. In höheren Dosierungen kann es jedoch zu ausgedehnten Blockaden im Myokard mit toxischen Wirkungen auf die Funktion der Muskelzellen kommen. Bis zur Einführung der Lipidemulsionen als Behandlungsoption bei einer Lokalanästhetikaintoxikation war eine prolongierte kardio-pulmonale Reanimation die einzige therapeutische Option.
In dieser Arbeit wurden die aktuell häufig verwendeten langwirksamen Substanzen Bupivacain und Ropivacain hinsichtlich einer divergierenden Kardiotoxizität verglichen und die Präinkubation mit Lipofundin® als Möglichkeit zur Reduktion der Kardiotoxizität getestet.
Mit Zustimmung der Ethikkommission wurden insgesamt 46 humane Herzmuskelbündel in einem Organbad untersucht.
Es erfolgte im Abstand von drei Minuten eine Inkubation mit Koffein in steigenden Konzentrationen (0,5; 1,0; 1,5; 2,0; 3,0; 4,0 mmol/l) und nachfolgend die Applikation von handelsüblichen Lösungen von Lokalanästhetika
(50 – 100 – 150 µl). Es wurden entweder Bupivacain (0,25, 0,5, 0,75 mg), Ropivacain (0,375, 0,75, 1,125 mg) oder Ropivacain in Kombination mit Lipofundin® 20% N (200µl in Verdünnung 1:100) appliziert. Abschließend erfolgte die hochdosierte Gabe von Koffein (Gesamtkonzentration 32 mmol/l), um eine maximale Kontraktur auszulösen.
Es zeigte sich kein signifikanter Unterschied zwischen Bupivacain und Ropivacain hinsichtlich der Kontraktionsamplitude und der Grundspannung bei ähnlichem Verhalten in beiden Gruppen. Die Präinkubation mit Lipofundin® ergab im Vergleich zur Kontrollgruppe keinen signifikanten Effekt.
Eine signifikante konzentrationsabhängige Abnahme der Kontraktionsamplitude und der Grundspannung zeigte sich während der initialen Inkubation mit Koffein.
Initiale Schädigungen, passagere Ermüdungserscheinungen oder hypoxische Stoffwechselzustände könnten trotz bestmöglicher Bedingungen die Ergebnisse beeinträchtigt haben. Die vorliegenden in-vitro-Ergebnisse lassen sich nur limitiert auf in-vivo-Bedingungen übertragen und verlangen nach weiteren Studien, die eine differenzierte und präzise Wirkweise sowie Dosierung von Lipidemulsionen evaluieren.
Postoperative Übelkeit, postoperatives Erbrechen und/oder die Kombination aus beidem (PÜE) stellen seit vielen Jahren ein komplexes Problem dar, welches bei operativen Eingriffen in Allgemein- und Regionalanästhesie auftreten kann. Die Bezeichnung postanästhesiologische Übelkeit und/oder Erbrechen wäre daher ein treffenderer Begriff. Die vorliegende Arbeit untersuchte zum ersten Mal, inwieweit die Wirksamkeit von drei verschiedenen Antiemetika und drei antiemetischen Strategien von individuellen Risikofaktoren der Patienten abhängt. Des Weiteren wurde die Wirksamkeit der Kombinationen der eingesetzten Antiemetika untersucht. In einer großen randomisierten kontrollierten Studie mit mehrfach faktoriellem Design wurden 5002 Patienten mit erhöhtem Risiko für PÜE für sechs verschiedene Faktoren randomisiert: Propofol gegenüber volatilen Anästhetika, Lachgas versus Luft und Remifentanil versus Fentanyl, Ondansetron gegenüber Kontrolle, Dexamethason versus Kontrolle und Droperidol versus Kontrolle. Dies führte zu einer Gesamtzahl von 64 Kombinationen. Alle eingeschlossenen Patienten wurden auf die Zielkriterien postoperative Übelkeit (PÜ), postoperative Emesis (PE) und postoperative Übelkeit oder Würgen oder Erbrechen (PÜWE) untersucht. Die Auswertungen dieser Zielvariablen erfolgten jeweils für ein frühes (bis zweite Stunde postoperativ), ein spätes Intervall (dritte bis 24. Stunde postoperativ) sowie den gesamten Beobachtungszeitraum von 24 Stunden. Als wichtigste Risikofaktoren für die Nebenwirkungen postoperative Übelkeit, Würgen und Erbrechen nach einer Allgemeinanästhesie zeigten sich das weibliche Geschlecht, die Verwendung von postoperativen Opioiden, der Nichtraucherstatus und die Narkosedauer. Dies galt für den Zeitraum bis zwei Stunden nach Narkose, von der dritten bis 24. Stunde und den gesamten Beobachtungszeitraum. Im Rahmen der PÜE-Anamnese eines Patienten stellte sich ein Quotient aus der Anzahl der PÜE bei Vornarkosen dividiert durch die Anzahl der stattgehabten Narkosen als signifikanter Prädiktor für PÜWE heraus, wenn ein Wert von 0,25 überschritten wurde. Dieser Quotient wurde erstmals ermittelt, um sowohl das Vorliegen von Vornarkosen als auch die Häufigkeit von postoperativer Übelkeit und/oder Erbrechen in der Anamnese mit einzubeziehen. Der Body-mass-Index ergab kein klinisch relevantes Risiko. Eine signifikante Reduktion von PÜWE, PÜ oder PE konnte durch die Verwendung von Propofol und/oder Luft gegenüber Narkosegas und Lachgas vor allem in der postoperativen Frühphase erreicht werden. Die Wahl des Opioides zur intraoperativen Analgesie spielte bezüglich der Inzidenz der untersuchten Nebenwirkungen keine Rolle. Die Antiemetika Ondansetron, Dexamethason und Droperidol waren sowohl als Mono- als auch als Kombinationsprophylaxe wirksam, dabei wirkten bezüglich des 24-Stunden-Intervalls für PÜWE die Kombinationen besser als die Monoprophylaxe, die Dreifachkombination war besser wirksam als die Zweifachkombinationen. Dexamethason bot für alle Nebenwirkungen eine schlechtere Wirksamkeit im frühen aber bessere Wirksamkeit im späten postoperativen Intervall. Demgegenüber zeigte Droperidol für alle Nebenwirkungen im frühen postoperativen Intervall eine bessere antiemetische Wirksamkeit gegenüber dem späten postoperativen Intervall. Als wichtigstes Ergebnis wurde festgestellt, dass zwischen den antiemetischen Interventionen und den individuellen Risikofaktoren keine Interaktionen vorlagen. Eine Ausnahme ergab sich jedoch im Hinblick auf die Wirkung von Droperidol und dem Geschlecht: bei Männern konnte für alle untersuchten Zeiträume keine signifikante Risikoreduktion für PÜ und PÜWE festgestellt werden. Zum ersten Mal zeigte dies, dass Droperidol bei Männern offenbar keine antiemetische Wirkung hatte, was jedoch in einer separaten Studie bestätigt werden sollte, da es klinisch relevante Konsequenzen für die Auswahl von Antiemetika in Abhängigkeit vom Geschlecht haben könnte.
Der Schlaganfall ist eine Krankheit mit großer Bedeutung, sowohl für die Betroffenen wie auch unter volkswirtschaftlichen Gesichtspunkten. In der Erforschung neuer und besserer Therapiemethoden für den ischämischen Schlaganfall ist ein gutes in-vitro-Modell der Blut-Hirn-Schranke unerlässlich, da ein Teil der Schädigung des ZNS durch einen Zusammenbruch dieser Barriere verursacht wird.
Die hCMEC/D3-Zelllinie stellt ein solches Modell dar; mit steigender Dauer der ischämischen Stoffwechsellage zeigt sich eine Erhöhung der LDH-Konzentration als Marker für das Absterben der Zellen sowie ein Rückgang der Zellvitalität. Zudem lässt sich eine Entzündungsreaktion mit Anstieg der Marker TNF-Alpha und VEGF, sowie tendenziell auch von Interleukin 6 und Interleukin 8 beobachten, welche auch auf eine Barriereschwächung hindeutet. Aus vorherigen Versuchen bekannte Tight junctions-Proteine wie Claudin 1 und Occludin waren in D3-Zellen unter ischämischen Bedingungen nicht verändert, Claudin 5 war in der PCR vermindert exprimiert. Die für die Barriereschwächung verantwortlichen Strukturproteine müssen durch weitere Versuche identifiziert werden. Eine mögliche Erhöhung der Expression des Transkriptionsfaktors ZO-1 könnte unter diesen Bedingungen einen Mechanismus der Barriereschwächung darstellen.
Die Expression des Glukokortikoidrezeptors war in Monokultur-Versuchen mit D3-Zellen nach Ischämie erniedrigt. Dies stellt eine Gemeinsamkeit mit Versuchen mit Zelllinien tierischen Ursprungs dar; in diesen zeigten die Zellen durch Degradation des Glukokortikoidrezeptors ein fehlendes Ansprechen auf eine Glukokortikoid-Behandlung. In der Cokultur der D3-Zellen mit Gliomzellen der C6-Zelllinie zeigte sich jedoch eine Erhöhung der GR-Expression. Eine Cokultur kann den komplexen Aufbau der Blut-Hirn-Schranke, mit Beteiligung mehrerer Zelltypen, besser darstellen als Versuche mit nur einer Zelllinie. Die Erhöhung der GR-Expression in diesem humanen in-vitro-Modell der Blut-Hirn-Schranke steht im Gegensatz zu den in-vitro-Versuchen mit anderen Zelllinien. Dies könnte eine mögliche Erklärung liefern, warum die Erkenntnisse aus diesen Versuchen bisher nicht zu einer Verbesserung der Evidenz der Glukokortikoid-Therapie beim ischämischen Schlaganfall beigetragen haben. Zudem zeigt die Fluoreszenzfärbung von D3-Zellen, dass diese auch unter Ischämie auf Glukokortikoide reagieren.
Background
Patients with cardiac arrest have lower survival rates, when resuscitation performance is low. In In-hospital settings the first responders on scene are usually nursing staff without rhythm analysing skills. In such cases Automated External Defibrillators (AED) might help guiding resuscitation performance. At the Wuerzburg University Hospital (Germany) an AED-program was initiated in 2007.
Aim of the presented study was to monitor the impact of Automated External Defibrillators on the management of in-hospital cardiac arrest events.
Methods
The data acquisition was part of a continuous quality improvement process of the Wuerzburg University Hospital. For analysing the CPR performance, the chest compression rate (CCR), compression depth (CCD), the no flow fraction (NFF), time interval from AED-activation to the first compression (TtC), the time interval from AED-activation to the first shock (TtS) and the post schock pause (TtCS) were determined by AED captured data. A questionnaire was completed by the first responders.
Results
From 2010 to 2012 there were 359 emergency calls. From these 53 were cardiac arrests with an AED-application. Complete data were available in 46 cases. The TtC was 34 (32–52) seconds (median and IQR).The TtS was 30 (28–32) seconds (median and IQR) . The TtCS was 4 (3–6) seconds (median and IQR) . The CCD was 5.5 ± 1 cm while the CCR was 107 ± 11/min. The NFF was calculated as 41 %.
ROSC was achieved in 21 patients (45 %), 8 patients (17 %) died on scene and 17 patients (37 %) were transferred under ongoing CPR to an Intensive Care Unit (ICU).
Conclusion
The TtS and TtC indicate that there is an AED-user dependent time loss. These time intervals can be markedly reduced, when the user is trained to interrupt the AED’s “chain of advices” by placing the electrode-paddles immediately on the patient’s thorax. At this time the AED switches directly to the analysing mode. Intensive training and adaption of the training contents is needed to optimize the handling of the AED in order to maximize its advantages and to minimize its disadvantages.
Mass critical care caused by the severe acute respiratory syndrome corona virus 2 pandemic poses an extreme challenge to hospitals. The primary goal of hospital disaster preparedness and response is to maintain conventional or contingency care for as long as possible. Crisis care must be delayed as long as possible by appropriate measures. Increasing the intensive care unit (ICU) capacities is essential. In order to adjust surge capacity, the reduction of planned, elective patient care is an adequate response. However, this involves numerous problems that must be solved with a sense of proportion. This paper summarises preparedness and response measures recommended to acute care hospitals.
Purpose
Rescue missions during terrorist attacks are extremely challenging for all rescue forces (police as well as non-police forces) involved. To improve the quality and safety of the rescue missions during an active killing event, it is obligatory to adapt common rescue mission goals and strategies.
Methods
After the recent attacks in Europe, the Federal Office of Civil Protection and Disaster Assistance started an evaluation process on behalf of the Federal Ministry of the Interior and the Federal Ministry of Health. This was done to identify weaknesses, lessons learned and to formulate new adapted guidelines.
Results
The presented bullet point recommendations summarise the basic and most important results of the ongoing evaluation process for the Federal Republic of Germany. The safety of all the rescue forces and survival of the greatest possible number of casualties are the priority goals. Furthermore, the preservation and re-establishment of the socio-political integrity are the overarching goals of the management of active killing events. Strategic incident priorities are to stop the killing and to save as much lives as possible. The early identification and prioritised transportation of casualties with life-threatening non-controllable bleeding are major tasks and the shortest possible on-scene time is an important requirement with respect to safety issues.
Conclusion
With respect to hazard prevention tactics within Germany, we attributed the highest priority impact to the bullet points. The focus of the process has now shifted to intense work about possible solutions for the identified deficits and implementation strategies of such solutions during mass killing incidents.
Background. Missed or delayed detection of progressive neuronal damage after traumatic brain injury (TBI) may have negative impact on the outcome. We investigated whether routine follow-up CT is beneficial in sedated and mechanically ventilated trauma patients. Methods. The study design is a retrospective chart review. A routine follow-up cCT was performed 6 hours after the admission scan. We defined 2 groups of patients, group I: patients with equal or recurrent pathologies and group II: patients with new findings or progression of known pathologies. Results. A progression of intracranial injury was found in 63 patients (42%) and 18 patients (12%) had new findings in cCT 2 (group II). In group II a change in therapy was found in 44 out of 81 patients (54%). 55 patients with progression or new findings on the second cCT had no clinical signs of neurological deterioration. Of those 24 patients (44%) had therapeutic consequences due to the results of the follow-up cCT. Conclusion. We found new diagnosis or progression of intracranial pathology in 54% of the patients. In 54% of patients with new findings and progression of pathology, therapy was changed due to the results of follow-up cCT. In trauma patients who are sedated and ventilated for different reasons a routine follow-up CT is beneficial.
Background:
Until now there has been a reported lack of systematic reports and scientific evaluations of rescue missions during terror attacks. This however is urgently required in order to improve the performance of emergency medical services and to be able to compare different missions with each other. Aim of the presented work was to report the systematic evaluation and the lessons learned from the response to a terror attack that happened in Wuerzburg, Germany in 2016.
Methods:
A team of 14 experts developed a template of quality indicators and operational characteristics, which allow for the description, assessment and comparison of civil emergency rescue missions during mass killing incidents. The entire systematic evaluation process consisted of three main steps. The first step was the systematic data collection according to the quality indicators and operational characteristics. Second was the systematic stratification and assessment of the data. The last step was the prioritisation of the identified weaknesses and the definition of the lessons learned.
Results:
Five important “lessons learned” have been defined. First of all, a comprehensive concept for rescue missions during terror attacks is essential. Furthermore, the establishment of a defined high priority communication infrastructure between the different dispatch centres (“red phone”) is vital. The goal is to secure the continuity of information between a few well-defined individuals. Thirdly, the organization of the incident scene needs to be commonly decided and communicated between police, medical services and fire services during the mission. A successful mission tactic requires continuous flux of reports to the on-site command post. Therefore, a predefined and common communication infrastructure for all operational forces is a crucial point. Finally, all strategies need to be extensively trained before the real life scenario hits.
Conclusion:
According to a systematic evaluation, we defined the lessons learned from a terror attack in 2016. Further systematic reports and academic work surrounding life threatening rescue missions and mass killing incidents are needed in order to ultimately improve such mission outcomes. In the future, a close international collaboration might help to find the best database to report and evaluate major incidents but also mass killing events.
Objective: To investigate the impact of the phophodiesterase-4 inhibition (PD-4-I) with rolipram on hepatic integrity in lipopolysaccharide (LPS) induced hyperinflammation. Materials and Methods: Liver microcirculation in rats was obtained using intravital microscopy. Macrohemodynamic parameters, blood assays, and organs were harvested to determine organ function and injury. Hyperinflammation was induced by LPS and PD-4-I rolipram was administered intravenously one hour after LPS application. Cell viability of HepG2 cells was measured by EZ4U-kit based on the dye XTT. Experiments were carried out assessing the influence of different concentrations of tumor necrosis factor alpha (TNF-α) and LPS with or without PD-4-I. Results: Untreated LPS-induced rats showed significantly decreased liver microcirculation and increased hepatic cell death, whereas LPS + PD-4-I treatment could improve hepatic volumetric flow and cell death to control level whithout influencing the inflammatory impact. In HepG2 cells TNF-α and LPS significantly reduced cell viability. Coincubation with PD-4-I increased HepG2 viability to control levels. The heme oxygenase 1 (HO-1) pathway did not induce the protective effect of PD-4-I. Conclusion: Intravenous PD-4-I treatment was effective in improving hepatic microcirculation and hepatic integrity, while it had a direct protective effect on HepG2 viability during inflammation.
Hintergrund: Durch den Einsatz der Hochfrequenzoszillationsbeatmung (HFOV) kann das applizierte Tidalvolumen minimiert und dadurch das Risiko für alveolären Scherstress reduziert werden, allerdings resultieren höhere Oszillationsfrequenzen in einer insuffizienten CO2-Elimination mit Entstehung einer Hyperkapnie und respiratorischen Azidose. In dieser experimentellen Studie wurde die Auswirkung verschiedener Oszillationsfrequenzen bei der HFOV auf die CO2-Elimination mit und ohne die Hinzunahme einer arteriovenösen extrakorporalen Lungenassistenz (avECLA) im Großtier-ARDS-Modell untersucht. Unsere Hypothese: die Verwendung hoher Oszillationsfrequenzen und damit die Minimierung des Tidalvolumens erfordert die Kombination einer HFOV mit einer avECLA, um Normokapnie zu erhalten oder wiederherzustellen.
Methodik: Hierzu wurden acht gesunde Pietrain-Schweine (56,5 ± 4,4 kg) narkotisiert und intubiert und anschließend mittels pulmonaler Lavage ein schweres iatrogenes ARDS herbeigeführt. Nach einstündiger Stabilisierungsphase (PaO2 durchgehend < 80 mmHg) erfolgte ein Recruitment-Manöver und die Einstellung des mittleren Atemwegsdrucks auf 3 cmH2O über dem zuvor bestimmten unteren Inflektionspunkt. Anschließend wurden die Tiere der HFOV zugeführt, randomisiert und mit entweder auf- oder absteigenden Oszillationsfrequenzen jeweils 30 Minuten mit und ohne Hinzunahme der avECLA beatmet.
Ergebnisse: Ab Oszillationsfrequenzen von 9 Hz entwickelten die Versuchstiere ohne die Hinzunahme einer avECLA zügig eine Hyperkapnie, welcher nur durch die Hinzunahme der avECLA entgegengewirkt werden konnte. Durch das Recruitment-Manöver und die Einstellung des mittleren Atemwegsdrucks auf 3 cmH2O über dem unteren Inflektionspunkt konnte die Oxygenierung dauerhaft signifikant verbessert werden (p<0.05). Die Ergebnisse der beiden Gruppen (auf- vs. absteigende Oszillationsfrequenzen) unterschieden sich dabei nicht voneinander.
Zusammenfassung: Bei der Hochfrequenzoszillationsbeatmung (HFOV) konnte Normokapnie bei Oszillationsfrequenzen von 9 – 15 Hz nur durch die Kombination mit einer arteriovenösen extrakorporalen Lungenassistenz (avECLA) aufrecht erhalten werden. Zusätzlich konnte nach dem Recruitment-Manöver und der Einstellung des mittleren Atemwegsdrucks auf 3 cmH2O über dem unteren Inflektionspunkt auch noch bei sehr hohen Oszillationsfrequenzen eine dauerhafte, signifikante Verbesserung der Oxygenierung verzeichnet werden. Somit demaskiert die avECLA das lungenprotektive Potential der HFOV: die Minimierung der applizierten Tidalvolumina begrenzt nicht nur eine alveoläre Überblähung und damit Volutraumata, die Applikation höherer mittlerer Atemwegsdrücke ermöglicht darüber hinaus ein pulmonales Recruitment und schützt die Lunge damit vor Atelekttraumata.
Kritisch-Kranke Patienten auf der Intensivstation unterliegen einem Postaggressionsstoffwechsel. Die Ernährungstherapie kritisch kranker Patienten ist ein wesentlicher Pfeiler im Therapiekonzept dieser Patienten und hat Auswirkung auf deren Outcome. Es wurde in der vorliegenden Arbeit das Ernährungsregime der anästhesiologischen Intensivstation untersucht, deren Nährstoffversorgung, Outcome sowie die damit verbundenen Kosten.
Trotz des breiten, klinischen Einsatzes des Propofols haben sich aufgrund bestehender Nebenwirkungen zunehmend Bestrebung entwickelt, alternative Formulierungen zur Lipidemulsion zu finden. In dieser Arbeit wurden die Substanzen Propofol/HPβCD und Na-Propofolat/HPβCD als alternative Formulierungen des Propofols mittels 1H-NMR-Spektroskopie, molekularer Modellierungen, DSC-Messungen, Zytotoxizitätstests (MTT-Test, LDH-Assay, EZ4U-Test) und dem Comet-Assay als Genotoxizitätstest auf biologische, chemische und physikalische Eigenschaften untersucht. Die Frage nach der Komplexbildung zwischen Propofol und HPβCD lässt sich nicht allein auf Basis des 1D-1H-NMR-Spektrums beantworten. Allerdings lässt sich mit Hilfe der 1H-NMR-Spektroskopie für Na-Propofolat/HPβCD das Vorliegen als deprotonierte Form nachweisen. Anderseits legen aber die DSC-Messungen eine Komplexbildung zwischen Propofol und HPβCD nahe. Zum einen ergibt sich das aus den unterschiedlichen Glasübergangstemperaturen der Substanzen. Zum anderen zeigt der Kurvenverlauf für HPβCD als auch für Na-Propofolat/HPβCD und Propofol/HPβCD keinen Verdampfungspeak bis zu der untersuchten maximalen Temperatur von 280 °C trotz des Propofol-Siedepunktes von 256 °C. Die molekularen Modellierungen legen nahe, dass die Bindung zwischen Propofol und Cyclodextrin über die Isopropylgruppen des Propofols erfolgt, wobei der aromatische Ring nicht in die Cavität des Cyclodextrinringes eindringt. Aus den molekularen Modellierungen ergeben sich Enthalpiewerte, die in ihrer Größenordnung schwachen Wasserstoffbrückenbindungen entsprechen. Für das Na-Propofolat/HPβCD lässt sich eine leichtere Abspaltung des Propofols aus dem Komplex voraussagen, wohingegen für Propofol/HPβCD die Abspaltung langsamer, aber über einen längeren Zeitraum erfolgen dürfte. Im Vergleich zu der hohen Zytotoxizität des Propofols als Lipidemulsion ergibt sich aus den an der CerebEND-Zelllinie durchgeführten Zyotoxizitätstests für HPβCD, Propofol/HPβCD und Na-Propofolat/HPβCD kein Nachweis einer Zytotoxizität nach 24-stündiger Exposition. Die Resultate zeigen für die drei Testverfahren die jeweils gleiche Reihenfolge mit der höchsten Zellvitalität für die mit Na-Propofolat/HPβCD behandelten Zellen und der niedrigsten Zellvitalität für HPβCD. In dem Comet-Assay zeigt sich nach 24-stündiger Exposition bei 37 °C für HPβCD, Propofol/HPβCD und Na-Propofolat/HPβCD keine genotoxische Wirkung an der HL-60-Zelllinie. Die Ergebnisse dieser Arbeit sprechen für die Fortsetzung der Untersuchungen von Propofol/HPβCD und Na-Propofolat/HPβCD als aussichtsreiche Option der Propofol-Formulierungen.
Propofol is a widely used general anesthetic in clinical practice, but its use is limited by its water-insoluble nature and associated pharmacokinetic and pharmacodynamic limitations. Therefore, researchers have been searching for alternative formulations to lipid emulsion to address the remaining side effects. In this study, novel formulations for propofol and its sodium salt Na-propofolat were designed and tested using the amphiphilic cyclodextrin (CD) derivative hydroxypropyl-β-cyclodextrin (HPβCD). The study found that spectroscopic and calorimetric measurements suggested complex formation between propofol/Na-propofolate and HPβCD, which was confirmed by the absence of an evaporation peak and different glass transition temperatures. Moreover, the formulated compounds showed no cytotoxicity and genotoxicity compared to the reference. The molecular modeling simulations based on molecular docking predicted a higher affinity for propofol/HPβCD than for Na-propofolate/HPβCD, as the former complex was more stable. This finding was further confirmed by high-performance liquid chromatography. In conclusion, the CD-based formulations of propofol and its sodium salt may be a promising option and a plausible alternative to conventional lipid emulsions.
We report on a nine-month-old girl who presented with persisting cough, and diminished ventilation of the left hemithorax. Viral pneumonia was suspected after Adenovirus detection by PCR, but chest X-rays showed a persistent shadowing of the left hemithorax and persistent coughing despite clinical improvement. Because of the discrepancy between clinical and radiological signs further investigations by ultrasound and CT scan were performed, which visualized an intrathroracic tumor. Histopathology confirmed diagnosis of a teratoma.
This case highlights the need for careful evaluation by the treating physicians. If the chest X-ray provides a discrepancy to the clinical findings or persistent pathologies exist, differential diagnosis should be discussed and further diagnostics be performed.
Backround: In February 2021, the first formal evidence and consensus-based (S3) guidelines for the inpatient treatment of patients with COVID-19 were published in Germany and have been updated twice during 2021. The aim of the present study is to re-evaluate the dissemination pathways and strategies for ICU staff (first evaluation in December 2020 when previous versions of consensus-based guidelines (S2k) were published) and question selected aspects of guideline adherence of standard care for patients with COVID-19 in the ICU. Methods: We conducted an anonymous online survey among German intensive care staff from 11 October 2021 to 11 November 2021. We distributed the survey via e-mail in intensive care facilities and requested redirection to additional intensive care staff (snowball sampling). Results: There was a difference between the professional groups in the number, selection and qualitative assessment of information sources about COVID-19. Standard operating procedures were most frequently used by all occupational groups and received a high quality rating. Physicians preferred sources for active information search (e.g., medical journals), while nurses predominantly used passive consumable sources (e.g., every-day media). Despite differences in usage behaviour, the sources were rated similarly in terms of the quality of the information on COVID-19. The trusted organizations have not changed over time. The use of guidelines was frequently stated and highly recommended. The majority of the participants reported guideline-compliant treatment. Nevertheless, there were certain variations in the use of medication as well as the criteria chosen for discontinuing non-invasive ventilation (NIV) compared to guideline recommendations. Conclusions: An adequate external source of information for nursing staff is lacking, the usual sources of physicians are only appropriate for the minority of nursing staff. The self-reported use of guidelines is high.
Die Magen-Darm-Motilität von Patienten auf Intensivstationen unterliegt vielen hemmenden Einflüssen. Außer der Wirkung inhibitorisch wirksamer Pharmaka wäre denkbar, dass auch organische Lösungsmittel (Ethanol, DMSO), Detergentien (SLS/CAPB) und aus Perfusor- und Infusionsleitungen freigesetzte Weichmacher (Phthalate) die Dünndarmperistaltik per se beeinflussen oder die Wirkung inhibitorisch wirksamer Pharmaka (Midazolam, Fentanyl) modulieren. Die Untersuchungen wurden in vitro am Dünndarm des Meerschweinchens durchgeführt. Der Parameter zur Beurteilung der inhibitorischen Wirkung auf die Peristaltik einer dem Organbad zugegebenen Substanz war die Änderung des intraluminalen Schwellendrucks ΔPPT (peristaltic pressure threshold) zur Auslösung von Peristaltik. Ein Anstieg der PPT zeigt eine inhibitorische Wirkung an. In der Zusammenschau der Ergebnisse aller Experimente der vorliegenden Arbeit zeigte sich, dass weder DEHP, Ethanol, DMSO noch Detergentien (SLS/CAPB) per se eine konzentrationsabhängige, signifikant hemmende Wirkung auf die Dünndarmperistaltik haben. Jedoch vermochten Ethanol, sowohl extraserosal dem Organbad zugegeben als auch endoluminal durch das Dünndarmsegment perfundiert, und in Ethanol gelöstes DEHP die motilitätshemmende Wirkung von Midazolam bzw. Fentanyl zu verstärken. In den Untersuchungen fiel auf, dass die Schwellendruckänderungen ΔPPT nach Zugabe von Ethanol, unabhängig von der jeweiligen Konzentration, sehr heterogen waren. Um den Mechanismus der Ethanolwirkung genauer zu charakterisieren, wurden Darmsegmente vor der Zugabe von Ethanol mit Antagonisten bzw. Blockern vermuteter Signaltransduktionswege vorbehandelt. Eingesetzt wurden Naloxon (Antagonist an Opioidrezeptoren), Apamin (Inhibitor von calciumaktivierten small conductance Kaliumkänalen), Bicucullin (Antagonist am GABAA-Rezeptor), Lorglumid (Antagonist am Cholecystokinin CCKA-Rezeptor) und YM022 (selektiver Antagonist am Gastrin/CCKB-Rezeptor). Diese Antagonisierungsversuche ergaben keine signifikanten Ergebnisse, da Ethanol in der Konzentration mit inhibitorischer Wirkung auf die Peristaltik zu heterogene Änderungen der PPT hervorrief. Unter klinischen Gesichtspunkten könnten die hemmende Wirkung von Ethanol auf die Peristaltik sowie die Wirkungsverstärkung des motilitätshemmenden Midazolam und Fentanyl durch Ethanol bzw. in Ethanol gelöstes DEHP Faktoren sein, die zur Hemmung der Darmmotilität bei Intensivpatienten beitragen.
Eine schwerwiegende Nebenwirkung aller Opioide in der Therapie akuter und chronischer Schmerzen ist die Hemmung der Darmmotilität. In der vorliegenden Arbeit wird untersucht, ob und ggf. über welche Mechanismen die nichtopioidergen Analgetika Paracetamol, Metamizol, Acetylsalicylsäure und das partiell opioiderge Tramadol die Darmperistaltik beeinflussen. Die Experimente wurden an Dünndarmsegmenten des Meerschweinchens in vitro durchgeführt. Ein wesentliches Ergebnis der Arbeit ist, dass Paracetamol die Dünndarmperistaltik konzentrationsabhängig vorübergehend hemmt. Durch Vorbehandlung mit Antagonisten und Inhibitoren der vermuteten Signaltransduktionswege wurden die Mechanismen der Hemmung untersucht. Durch Naloxon und Apamin konnte die hemmende Wirkung von Paracetamol reduziert werden, was zeigt, dass enterische opioiderge Transduktionswege sowie eine Aktivierung von small conductance Ca2+-activated Kaliumkanälen beteiligt sind. Enterisches Stickstoffmonoxid (NO), die Cyclooxygenase und Serotonin spielen dabei als Transduktionsmechanismen keine Rolle. Metamizol und Acetylsalicylsäure haben keinen hemmenden Einfluß auf die Dünndarmperistaltik. Razemisches Tramadol, die beiden Enantiomere (+)- und (-)-Tramadol sowie der Hauptmetabolit O-Desmethyltramadol hemmen konzentrationsabhängig die Darmmotilität; hierbei wirkt (+)-Tramadol stärker als (-)-Tramadol, beide aber deutlich geringer als O-Desmethyltramadol.. An der Wirkung von (+)- und (-)-Tramadol sind opioiderge und adrenerge Mechanismen beteiligt, die Wirkung von O-Desmethyltramadol wird durch Bindung an Opiatrezeptoren vermittelt.
Evidence synthesis findings depend on the assumption that the included studies follow good clinical practice and results are not fabricated or false. Studies which are problematic due to scientific misconduct, poor research practice, or honest error may distort evidence synthesis findings. Authors of evidence synthesis need transparent mechanisms to identify and manage problematic studies to avoid misleading findings. As evidence synthesis authors of the Cochrane COVID-19 review on ivermectin, we identified many problematic studies in terms of research integrity and regulatory compliance. Through iterative discussion, we developed a research integrity assessment (RIA) tool for randomized controlled trials for the update of this Cochrane review. In this paper, we explain the rationale and application of the RIA tool in this case study. RIA assesses six study criteria: study retraction, prospective trial registration, adequate ethics approval, author group, plausibility of methods (e.g., randomization), and plausibility of study results. RIA was used in the Cochrane review as part of the eligibility check during screening of potentially eligible studies. Problematic studies were excluded and studies with open questions were held in awaiting classification until clarified. RIA decisions were made independently by two authors and reported transparently. Using the RIA tool resulted in the exclusion of >40% of studies in the first update of the review. RIA is a complementary tool prior to assessing “Risk of Bias” aiming to establish the integrity and authenticity of studies. RIA provides a platform for urgent development of a standard approach to identifying and managing problematic studies.
Objective
In this abridged version of the recently published Cochrane review on antiemetic drugs, we summarize its most important findings and discuss the challenges and the time needed to prepare what is now the largest Cochrane review with network meta-analysis in terms of the number of included studies and pages in its full printed form.
Methods
We conducted a systematic review with network meta-analyses to compare and rank single antiemetic drugs and their combinations belonging to 5HT₃-, D₂-, NK₁-receptor antagonists, corticosteroids, antihistamines, and anticholinergics used to prevent postoperative nausea and vomiting in adults after general anesthesia.
Results
585 studies (97 516 participants) testing 44 single drugs and 51 drug combinations were included. The studies’ overall risk of bias was assessed as low in only 27% of the studies. In 282 studies, 29 out of 36 drug combinations and 10 out of 28 single drugs lowered the risk of vomiting at least 20% compared to placebo. In the ranking of treatments, combinations of drugs were generally more effective than single drugs. Single NK1 receptor antagonists were as effective as other drug combinations. Of the 10 effective single drugs, certainty of evidence was high for aprepitant, ramosetron, granisetron, dexamethasone, and ondansetron, while moderate for fosaprepitant and droperidol. For serious adverse events (SAEs), any adverse event (AE), and drug-class specific side effects evidence for intervention effects was mostly not convincing.
Conclusions
There is high or moderate evidence for at least seven single drugs preventing postoperative vomiting. However, there is still considerable lack of evidence regarding safety aspects that does warrant investigation.
Laut der Weltgesundheitsorganisation (WHO) waren in Deutschland 2006 akute ischämische Ereignisse des Zentralen Nervensystems (ZNS) die fünfthäufigste Todesursache. Zu diesen ischämischen Ereignissen zählen Schlaganfall, Kardiopulmonale Reanimation, traumatische Hirnverletzungen, sowie perioperative ischämische Komplikationen. Aufgrund der schwerwiegenden Folgen, die ein Verlust von Nervenzellen für den Patienten bedeutet, muss die weitere medizinische Akutversorgung den sekundären neuronalen Schaden verhindern oder ihn reduzieren. Vor dieser Arbeit konnten Glukosetransporter-1 (GLUT-1) und Natrium-Glukose-Kotransporter-1 (SGLT1) an der Blut-Hirn-Schranke (BHS) identifiziert werden. Ziel dieser Arbeit war es, das Expressionsverhalten der Glukosetransporter nach einem Schädel-Hirn-Trauma (SHT) in vivo und in vitro zu untersuchen, um so den Einfluss und die funktionellen Folgen durch die veränderte Expression der zerebralen Glukosetransporter in der BHS infolge eines SHT zu identifizieren und deren eventuellen Einfluss auf die Entwicklung eines sekundären Hirnödems zu erkennen. Hierfür wurde als in vivo-Modell das Controlled Cortical Impact Injury (CCII) gewählt, da bei diesem Tierversuchsmodell die Aspekte der traumatischen Kontusion und die damit verbundenen intraparenchymalen Blutungen durch ein epidurales oder subdurales Hämatom im Vordergrund stehen. Es wurden Gehirnschnitte zu fest definierten Zeitpunkten angefertigt (kein CCII (Kontrolle), 15 Minuten Überleben nach CCII (Primärschaden), 24 Stunden Überleben nach CCII und 72 Stunden Überleben nach CCII). Die Darstellung des primären Schadens im Mäusehirn erfolgte durch die Immunfluoreszenzmikroskopie. Um einen Gewebeschaden, wie es bei einem Hirntrauma der Fall ist, in vitro zu simulieren, wurde das Modell des Sauerstoff-Glukose-Entzuges (OGD) gewählt, da es bei diesem Modell neben einer Nekrose auch zur Apoptose der Nervenzellen kommt, welche ebenfalls bei einem SHT stattfindet. Als geeignetes Zellkulturmodell wurde die cerebralen Endothelzelllinie (cEND) gewählt. Bei dieser Zelllinie handelte es sich um eine Hirnendothelzelllinie aus der Maus. In den in vivo-Versuchen konnte bei GLUT-1 bereits 15 Minuten nach CCII eine gesteigerte Expression festgestellt werden. Dennoch verminderte GLUT-1 im weiteren Verlauf seine Expression auf ein Minimum, welches unterhalb des Ausgangswertes lag. SGLT1, der auch in der BHS identifiziert wurde, reagierte auf einen Primärschaden erst in den Hirnschnitten, die 24 Stunden nach CCI behandelt wurden. In den Hirnschnitten, die 15 Minuten nach CCII behandelt wurden, veränderte sich die SGLT1-Expression zunächst nicht. Erst 24 Stunden nach CCII konnte eine gesteigerte Expression von SGLT1 erkannt werden, die aber bei 72 Stunden nach CCII wieder abgenommen hatte. Ein weiterer Glukosetransporter konnte erstmals in der BHS identifiziert werden. SGLT2 zeigte erst 72 Stunden nach CCII eine gesteigerte Expression, in den Hirnschnitten ohne CCII, 15 Minuten nach CCII und 24 Stunden nach CCII konnte keine Veränderung der SGLT2-Expression festgestellt werden. Diese Expressionsreaktion, besonders der Expressions-Höhepunkt der einzelnen Glukosetransporter, konnte auch in vitro gezeigt werden. Besonders die Identifizierung von SGLT2 in der BHS und die generelle Steigerung der Expressionsrate von GLUT-1, SGLT1 und SGLT2 könnte neue Ansatzpunkte in der Pathophysiologie des diffusen Hirnödems nach einem SHT ergeben. Die genaue Rolle der Natriumgekoppelten Glukosetransporter in der BHS muss noch weiter erforscht werden. Bestätigen weitere Versuche eine zentrale Rolle der SGLTs bei der Entstehung des sekundären Hirnschadens, speziell SGLT2, als hochpotenter Glukosetransporter, so könnte über neue Therapien nachgedacht werden, durch welche spezifisch die Expression der SGLTs, besonders SGLT2, wie es bei Dapagliflozin, Canagliflozin oder Ipragliflozin der Fall wäre, unterdrücken würden.
Background: The present anonymous multicenter online survey was conducted to evaluate the application of regional anaesthesia techniques as well as the used local anaesthetics and adjuncts at German and Austrian university hospitals. Methods: 39 university hospitals were requested to fill in an online questionnaire, to determine the kind of regional anaesthesia and preferred drugs in urology, obstetrics and gynaecology. Results: 33 hospitals responded. No regional anaesthesia is conducted in 47% of the minor gynaecological and 44% of the urological operations; plain bupivacaine 0.5% is used in 38% and 47% respectively. In transurethral resections of the prostate and bladder no regional anaesthesia is used in 3% of the responding hospitals, whereas plain bupivacaine 0.5% is used in more than 90%. Regional anaesthesia is only used in selected major gynaecological and urological operations. On the contrary to the smaller operations, the survey revealed a large variety of used drugs and mixtures. Almost 80% prefer plain bupivacaine or ropivacaine 0.5% in spinal anaesthesia in caesarean section. Similarly to the use of drugs in major urological and gynaecological operations a wide range of drugs and adjuncts is used in epidural anaesthesia in caesarean section and spontaneous delivery. Conclusions: Our results indicate a certain agreement in short operations in spinal anaesthesia. By contrast, a large variety concerning the anaesthesiological approach in larger operations as well as in epidural analgesia in obstetrics could be revealed, the causes of which are assumed to be primarily rooted in particular departmental structures.
Background
Aim of this study was to evaluate prognosis of severely injured patients.
Methods
All severely injured patients with an Injury Severity Score (ISS) ≥ 50 were identified in a 6-year-period between 2000 and 2005 in German Level 1 Trauma Center Murnau. Data was evaluated from German Trauma Registry and Polytrauma Outcome Chart of the German Society for Trauma Surgery and a personal interview to assess working ability and disability and are presented as average.
Results
88 out of 1435 evaluated patients after severe polytrauma demonstrated an ISS ≥ 50 (6.5%), among them 23% women and 77% men. 66 patients (75%) had an ISS of 50-60, 14 (16%) 61-70, and 8 (9%) ≥ 70. In 27% of patients trauma was caused by motor bike accidents. 3.6 body regions were involved. Patients had to be operated 5.3 times and were treated 23 days in the ICU and stayed 73 days in hospital. Mortality rate was 36% and rate of multi-organ failure 28%. 15% of patients demonstrated severe senso-motoric dysfunction as well as residues of severe head injury. 25% recovered well or at least moderately. 29 out of 56 survivors answered the POLO-chart. A personal interview was performed with 13 patients. The state of health was at least moderate in 72% of patients. In 48% interpersonal problems and in 41% severe pain was observed. In 57% of patients problems with working ability regarding duration, as well as quantitative and qualitative performance were observed. Symptoms of post-traumatic stress disorder were found in 41%. The more distal the lesions were located (foot/ankle) the more functional disability affected daily life. In only 15%, working ability was not impaired. 8 out of 13 interviewed patients demonstrated complete work disability.
Conclusions
Even severely injured patients after multiple trauma have a good prognosis. The ISS is an established tool to assess severity and prognosis of trauma, whereas prediction of clinical outcome cannot be deducted from this score.
Ob eine Immunonutrition einen Nutzen in der Versorgung schwer kranker oder chirurgischer Patienten bringt, ist Thema kontroverser Diskussionen. Dabei scheint unklar, welches Patientenklientel von welcher Zusammensetzung verschiedener immunmodulatorischer Substanzen am meisten profitiert. Zudem ist nicht einheitlich geklärt, zu welchen Zeitpunkten und über welche Zeiträume die Immunmodulation den größten Nutzen bringt. Die hier durchgeführte Studie wurde konzipiert, um zu untersuchen, ob die Patienten, die einer aortokoronaren Bypassoperation unter Einsatz der Herz-Lungen-Maschine zugeführt werden, eine Verbesserung ihres klinischen Outcomes zeigen, wenn sie immunmodulatorische Substanzen perioperativ erhalten. Die konkreten Fragestellungen für diese Arbeit lauten daher: 1) Kann die Immunonutrition mit Glutamin, ௰-3-Fettsäuren und antioxidativ wirksamen Vitaminen die Inzidenz von postoperativen Infektionen bei Patienten, die einer aortokoronaren Bypassoperation zugeführt werden, verringern? 2) Verkürzt sich die Liegedauer der Patienten auf der Intensivstation und nach Verlegung auf Normalstation postoperativ bei regelmäßiger Einnahme der immunmodulierenden Substanzen über den Zeitraum des stationären Aufenthaltes? 3) Verbessert sich das klinische Outcome der Patienten durch die perioperative Substitution insgesamt, durch ein selteneres Auftreten von häufigen Komplikationen und Kreislaufinstabilitäten? Zu diesem Zweck wurde eine Kohortenstudie mit 137 Patienten durchgeführt, wovon sich 97 Patienten in der Kontrollgruppe und 40 in der Experimentalgruppe befanden. Die Patienten der Experimentalgruppe erhielten ab dem Tag ihrer stationären Aufnahme 2 unterschiedliche Substanzen zur oralen Einnahme verabreicht. Die eine enthielt einen hohen Anteil an Glutamin und antioxidativ wirksamen Substanzen (Glutamine Plus®) und die andere einen hohen Gehalt an ௰-3-Fettsäuren (Supportan® Drink). Das Glutamine Plus® wurde morgens und abends, der Supportan® Drink mittags verabreicht. Die Substitution erfolgte über den kompletten stationären Aufenthalt, abgesehen von dem Tag der Operation selber. Die Blutentnahmen für die entsprechenden infektiologischen Parameter erfolgten zu den Zeitpunkten präoperativ, 6 Stunden nach Ende der Operation, 2 Tage postoperativ und vor der Entlassung. Es zeigte sich, dass die Patienten, die die Substanzen erhielten, weniger postoperative Infektionen entwickelt hatten, als die Patienten in der Kontrollgruppe. Das wurde aus einer signifikanten PCT-Erhöhung in der Kontrollgruppe abgeleitet. Allerdings hatte diese postoperative Komplikation keine Auswirkung auf die Liegezeit der Patienten auf der Intensivstation oder auf die Dauer des stationären Aufenthaltes nach Verlegung von der Intensivstation auf eine periphere Station zur Folge. Ebenfalls zeigten sich keine Unterschiede zwischen den beiden Kohorten in der Notwendigkeit einer antibiotischen Therapie, sowie im Auftreten von Kreislaufinstabilitäten oder den häufigsten Komplikationen.
Hintergrund: Patienten, die einen Herz-Kreislaufstillstand erleiden haben eine geringere Überlebensrate wenn Reanimationsmaßnahmen nicht suffizient durchgeführt werden. Innerhalb einer Klinik sind die Ersthelfer meistens Mitarbeiter der Pflege, die nur über geringe Kenntnisse in der Herzrhythmusanalyse verfügen. In solchen Fällen kann ein Automatischer Externer Defibrillator (AED) hilfreich sein um die Durchführung der Reanimationsmaßnahmen zu verbessern. Aus diesem Grund wurde im Jahr 2007 das AED-Programm am Universitätsklinikum Würzburg (Deutschland) eingeführt.
Ziel dieser Studie war die Auswirkungen der Automatischen Externen Defibrillatoren auf die innerklinischen Herz-Kreislaufstillstände und die Reanimationsmaßnahmen zu untersuchen.
Methodik: Die Datenerfassung erfolgte im Rahmen von Qualitätssicherungsmaßnahmen am Universitätsklinikum Würzburg. Daten zur Thoraxkompressionstiefe (CCD), Thoraxkompressionsfrequenz (CCR) und zur No-Flow-Fraktion (NFF), zum Zeitintervall vom Einschalten des AED bis zur ersten Thoraxkompression (TtC), zum Zeitintervall vom Einschalten des AED bis zur ersten Schockabgabe (TtS) und zum Zeitintervall nach Schockabgabe bis zum Beginn der Thoraxkompression (TtCS) wurden mit Hilfe der aufgezeichneten Daten des AED ausgewertet. Ein Fragebogen wurde von den Ersthelfern ausgefüllt.
Ergebnisse: Zwischen 2010 und 2012 wurden insgesamt 359 innerklinische Notrufe registriert. Davon waren 53 Herz-Kreislaufstillstände mit Gebrauch eines AED, in 46 Fällen waren vollständige Datensätze vorhanden. Die mittlere CCD lag bei 5,5 +/- 1 cm, die mittlere CCR betrug 107 +/- 11/min. Die NFF lag bei 41%.
Die TtC lag bei 34 (32-52) Sekunden (Median und IQR), die TtS betrug 30 (28-32) Sekunden (Median und IQR) und die TtCS lag bei 4 (3-6) Sekunden (Median und IQR).
21 Patienten (45%) erreichten einen ROSC, 8 Patienten (17%) verstarben vor Ort und 17 Patienten (37%) wurden unter laufender Reanimation auf eine Intensivstation (ITS) verlegt.
Zusammenfassung: Sowohl die TtC als auch die TtS verdeutlichen die durch den Anwender verursachte Zeitverzögerung. Diese Zeitintervalle können erheblich verkürzt werden sofern der Anwender die Sprachanweisungen des AED unterbricht und die Paddles direkt auf dem Thorax des Patienten anbringt. Der AED schaltet unmittelbar in den Analysemodus.
Häufige Schulungen der Mitarbeiter und regelmäßig angepasste Schulungsinhalte sind notwendig um den Umgang mit dem AED und somit die Reanimationsmaßnahmen zu verbessern.
Background
Even though bleeding and thromboembolic events are major complications of extracorporeal membrane oxygenation (ECMO), data on the incidence of venous thrombosis (VT) and thromboembolism (VTE) under ECMO are scarce. This study analyzes the incidence and predictors of VTE in patients treated with ECMO due to respiratory failure.
Methods
Retrospective analysis of patients treated on ECMO in our center from 04/2010 to 11/2015. Patients with thromboembolic events prior to admission were excluded. Diagnosis was made by imaging in survivors and postmortem examination in deceased patients.
Results
Out of 102 screened cases, 42 survivors and 21 autopsy cases [mean age 46.0 ± 14.4 years; 37 (58.7 %) males] fulfilling the above-mentioned criteria were included. Thirty-four patients (54.0 %) underwent ECMO therapy due to ARDS, and 29 patients (46.0 %) with chronic organ failure were bridged to lung transplantation. Despite systemic anticoagulation at a mean PTT of 50.6 ± 12.8 s, [VT/VTE 47.0 ± 12.3 s and no VT/VTE 53.63 ± 12.51 s (p = 0.037)], VT and/or VTE was observed in 29 cases (46.1 %). The rate of V. cava thrombosis was 15/29 (51.7 %). Diagnosis of pulmonary embolism prevailed in deceased patients [5/21 (23.8 %) vs. 2/42 (4.8 %) (p = 0.036)]. In a multivariable analysis, only aPTT and time on ECMO predicted VT/VTE. There was no difference in the incidence of clinically diagnosed VT in ECMO survivors and autopsy findings.
Conclusions
Venous thrombosis and thromboembolism following ECMO therapy are frequent. Quality of anticoagulation and ECMO runtime predicted thromboembolic events.

Viele im klinischen Alltag verwendete volatile Anästhetika verursachen während der Narkose eine Vasodilatation. In dieser Hinsicht wäre es interessant, zu Untersuchen, ob volatile Anästhetika die Prostacyclinbildung in Endothelzellen beeinflussen können. Für diese Untersuchungen wurden primäre humane Endothelzellen aus der Nabelschnurvene (HUVEC) isoliert und in Zellkulturen kultiviert. Diese Zellen zeigen nach Zugabe von Histamin eine Dosis- und Zeitabhängige Prostacyclinbildung. Entscheidend für diese Prostacyclinbildung ist die Ca2+-Freisetzung aus intrazellulären Speichern. Die dosisabhängige Untersuchung von Halothan auf die Prostacyclinbildung zeigte eine signifikante Stimulation der Prostacyclinbildung von 33,8% bei einer klinisch relevanten Konzentration von 1 Vol.%. Diese stimulierende Wirkung von Halothan auf die Prostacyclinbildung könnte einen Beitrag zu der in vivo beobachteten vasodilatierenden Wirkung des Anästhetikums leisten. Eine ähnliche stimulierende Wirkung wurde auch für Isofluran beobachtet, obwohl der stimulierende Effekt auf die Prostacyclinbildung keine statistische Relevanz erreichte. Höhere supraklinische Konzentrationen der beiden Anästhetika hemmen allerdings signifikant die Prostacyclinbildung. Die Aktivierung der Proteinkinase C in den HUVEC Zellen hat keinen signifikanten Einfluss auf die Histamin-induzierte Prostacyclinbildung. Dieses Ergebnis macht eine stimulierende Wirkung der volatilen Anästhetika auf die Prostacyclinbildung mittels Proteinkinase C-Aktivierung unwahrscheinlich. Die Stimulation der NO-Signalweges mittels Natrium-Nitroprussid in den HUVEC Zellen verursachte eine signifikante Hemmung der Histamin-induzierten Prostacyclinbildung. Andererseits führte die Hemmung des NO-Signalwegs mit L-NAME nicht zu einer signifikanten Zunahme der Prostacyclinbildung. Eine mögliche Hemmung des NO-Signalwegs durch volatile Anästhetika kann daher in HUVEC Zellen nicht durch vermehrte Prostacyclinbildung kompensiert werden.
Intracranial hemorrhage results in devastating forms of cerebral damage. Frequently, these results also present with cardiac dysfunction ranging from ECG changes to Takotsubo syndrome (TTS). This suggests that intracranial bleeding due to subarachnoid hemorrhage (SAH) disrupts the neuro–cardiac axis leading to neurogenic stress cardiomyopathy (NSC) of different degrees. Following this notion, SAH and secondary TTS could be directly linked, thus contributing to poor outcomes. We set out to test if blood circulation is the driver of the brain–heart axis by investigating serum samples of TTS patients. We present a novel in vitro model combining SAH and secondary TTS to mimic the effects of blood or serum, respectively, on blood–brain barrier (BBB) integrity using in vitro monolayers of an established murine model. We consistently demonstrated decreased monolayer integrity and confirmed reduced Claudin-5 and Occludin levels by RT-qPCR and Western blot and morphological reorganization of actin filaments in endothelial cells. Both tight junction proteins show a time-dependent reduction. Our findings highlight a faster and more prominent disintegration of BBB in the presence of TTS and support the importance of the bloodstream as a causal link between intracerebral bleeding and cardiac dysfunction. This may represent potential targets for future therapeutic inventions in SAH and TTS.
Advances in breast cancer management and extracellular vesicle research, a bibliometric analysis

Extracellular vesicles transport variable content and have crucial functions in cell–cell communication. The role of extracellular vesicles in cancer is a current hot topic, and no bibliometric study has ever analyzed research production regarding their role in breast cancer and indicated the trends in the field. In this way, we aimed to investigate the trends in breast cancer management involved with extracellular vesicle research. Articles were retrieved from Scopus, including all the documents published concerning breast cancer and extracellular vesicles. We analyzed authors, journals, citations, affiliations, and keywords, besides other bibliometric analyses, using R Studio version 3.6.2. and VOSviewer version 1.6.0. A total of 1151 articles were retrieved, and as the main result, our analysis revealed trending topics on biomarkers of liquid biopsy, drug delivery, chemotherapy, autophagy, and microRNA. Additionally, research related to extracellular vesicles in breast cancer has been focused on diagnosis, treatment, and mechanisms of action of breast tumor-derived vesicles. Future studies are expected to explore the role of extracellular vesicles on autophagy and microRNA, besides investigating the application of extracellular vesicles from liquid biopsies for biomarkers and drug delivery, enabling the development and validation of therapeutic strategies for specific cancers.
Hintergrund
Internationale Studien belegen negative Auswirkungen der COVID-19-Pandemie auf Stimmung und Stresslevel befragter Personen. Auch konnten Zusammenhänge zwischen der Pandemie und höheren Schmerzstärken sowie stärkerer schmerzbedingter Beeinträchtigung nachgewiesen werden. Die Studienlage dazu, ob ältere Menschen besser oder schlechter mit der Pandemie und ihren Auswirkungen umgehen können als jüngere Personen, ist aber uneindeutig.
Methodik
Seit einigen Jahren bietet das Universitätsklinikum Würzburg ein multimodales Schmerztherapieprogramm für SeniorInnen an. Für die vorliegende Arbeit wurden retrospektiv klinische Routinedaten zum Zeitpunkt des interdisziplinären multimodalen Assessments von n = 75 TeilnehmerInnen in den Jahren 2018 und 2019 mit denen von n = 42 Patientinnen während der COVID-19-Pandemie 2020–2021 verglichen. Wir untersuchten Schmerz, psychische Belastung und körperliches Funktionsniveau mithilfe des Deutschen Schmerzfragebogens, klinischer Diagnostik und geriatrischer Funktionstests.
Ergebnisse
Die beiden Teilstichproben unterschieden sich nicht in demografischen Merkmalen. Bezüglich Schmerzintensität und Beeinträchtigung sowie der psychischen Belastung fanden sich ebenfalls keine signifikanten Unterschiede. Lediglich die Anzahl der schmerzbedingt beeinträchtigten Tage war vor Corona signifikant höher. Die geriatrischen Funktionstests zeigten signifikant bessere Werte während der Pandemie an.
Diskussion
Die vorliegenden Daten zeigen keine Verschlechterung von Schmerz und körperlichem sowie psychischem Wohlbefinden bei SeniorInnen vor dem Hintergrund der Pandemie. Weitere Studien sollten die möglichen Gründe dafür untersuchen. Diese könnten in einer höheren Resilienz der SeniorInnen basierend auf ihrer Lebenserfahrung, finanziellen Sicherheit oder einer geringeren Veränderung des Lebensalltags liegen.
Die Maligne Hyperthermie (MH) ist eine metabolische Myopathie. Sie veranlagt zu einer potentiell lebensgefährlichen Stoffwechselentgleisung, die durch Exposition mit bestimmten Triggersubstanzen, insbesondere volatilen Anästhetika und depolarisierenden Muskelrelaxantien, ausgelöst wird. Dabei führt ein Defekt in der Membran des sarkoplasmatischen Retikulums der Skelettmuskulatur zur unkontrollierten Stoffwechselsteigerung. Zahlreiche Medikamente weisen in ihrem Nebenwirkungsprofil das Risiko von Rhabdomyolysen oder Myalgien auf. Unter Therapie von Cerivastatin kam es in 52 Fällen zu tödlichen Verläufen, so dass das Medikament 2001 vom Markt genommen wurde. Wir untersuchten Maligne Hyperthermie suszeptible Schweinemuskeln im In-vitro-Kontrakturtest (IVCT) unter Inkubation mit Atorvastatin, Simvastatin, Levofloxacin, Ciprofloxacin und Gemfibrozil. Für diese Medikamente sind derartige unerwünschte Arzneimittelwirkungen beschrieben. Wir konnten dabei zeigen, dass der Maligne Hyperthermie suszeptible (MHS) Muskel anscheinend empfindlicher auf Atorvastatin, Simvastatin, Levofloxacin und Ciprofloxacin reagiert als der Maligne Hyperthermie negative (MHN). Das zeigte sich anhand der gegenüber den nicht suszeptiblen Muskeln signifikant höher ansteigenden Ruhespannung der MHS-Muskelpräparate im IVCT. Wir erklären dieses Ergebnis durch die beim MHS-Muskel labilere Ca2+-Homöostase. Gemfibrozil scheint hingegen auf die Ca2+-Homöostase keinen Einfluss zu haben, jedoch in hohen Konzentrationen myotoxisch zu wirken. Eine Kombination mit anderen Medikamenten, die durch die gleichen Enzymsysteme abgebaut werden, sollte daher nur mit Vorsicht erfolgen, um eine Kumulation im Organismus zu vermeiden.
In der vorliegenden Studie wurde untersucht, ob der nichtkompetitive NMDA-Rezeptorantagonist Memantine die Wahrnehmung noxischer und nichtnoxischer Temperaturreize beim Menschen signifikant beeinflusst. Dazu wurden bei 40 Probanden, doppelblind und placebokontrolliert die Wahrnehmungsschwellen für Warm-, Kalt- und Hitzeschmerzreize bestimmt. Anschließend wurde ein noxischer Hitzereiz appliziert; die Schmerzintensität wurde aufgezeichnet. Danach wurden Veränderungen der Wahrnehmungsschwellen innerhalb und außerhalb des Reizareals registriert. Die Ausdehnung von Allodynie, sekundärer Hyperalgesie und Flarereaktion wurde vermessen. Bei der Memantinegruppe zeigte sich vor der Applikation noxischer Hitze eine signifikante Reduktion der Sensibilität für Kaltreize. Durch die Verabreichung des Hitzeschmerzreizes von 47°C wurden die Probanden beider Gruppen weniger sensibel gegenüber Warm- und Kaltreizen innerhalb der Hitzereizapplikationsstelle. Die Ausdehnung der Flarefläche und die Perfusion innerhalb des gereizten Areals waren bei Probanden durch die Memantinevorbehandlung deutlich reduziert. Aus diesen Ergebnissen lassen sich folgende Vermutungen ableiten: 1. Durch alleinige Blockade des NMDA-Rezeptors besteht bei chronischen Schmerzzuständen wenig Aussicht auf Schmerzlinderung. 2. Die Aktivierung des NMDA-Rezeptors ist für die Wahrnehmung von Kaltreizen von Bedeutung. 3. Ein Axonreflex löst die Flarereaktion nach Verabreichung eines noxischen Hitzereizes aus. Intensität und Ausdehnung der Flarereaktion werden durch NMDA-Rezeptoren moduliert.
Background: The adequate choice of perioperative antibiotic prophylaxis (PAP) could influence the risk of surgical site infections (SSIs) in general surgery. A new local PAP guideline was implemented in May 2017 and set the first-generation cefazolin (CFZ) instead the second-generation cefuroxime (CXM) as the new standard prophylactic antibiotic. The aim of this study was to compare the risk of SSIs after this implementation in intra-abdominal infections (IAIs) without sepsis. Methods: We performed a single center-quality improvement study at a 1500 bed sized university hospital in Germany analyzing patients after emergency surgery during 2016 to 2019 (n = 985), of which patients receiving CXM or CFZ were selected (n = 587). Propensity score matching was performed to ensure a comparable risk of SSIs in both groups. None-inferiority margin for SSIs was defined as 8% vs. 4%. Results: Two matched cohorts with respectively 196 patients were compared. The rate of SSIs was higher in the CFZ group (7.1% vs. 3.6%, p = 0.117) below the non-inferiority margin. The rate of other postoperative infections was significantly higher in the CFZ group (2.0% vs. 8.7%, p = 0.004). No other differences including postoperative morbidity, mortality or length-of-stay were observed. Conclusion: Perioperative antibiotic prophylaxis might be safely maintained by CFZ even in the treatment of intra-abdominal infections.
The blood-brain barrier (BBB) is a highly specialized structure that separates the brain from the blood and allows the exchange of molecules between these two compartments through selective channels. The breakdown of the BBB is implicated in the development of severe neurological diseases, especially stroke and traumatic brain injury. Oxygen-glucose deprivation is used to mimic stroke and traumatic brain injury in vitro. Pathways that trigger BBB dysfunction include an imbalance of oxidative stress, excitotoxicity, iron metabolism, cytokine release, cell injury, and cell death. MicroRNAs are small non-coding RNA molecules that regulate gene expression and are emerging as biomarkers for the diagnosis of central nervous system (CNS) injuries. In this review, the regulatory role of potential microRNA biomarkers and related therapeutic targets on the BBB is discussed. A thorough understanding of the potential role of various cellular and linker proteins, among others, in the BBB will open further therapeutic options for the treatment of neurological diseases.
Effect of Tjap1 knock-down on blood-brain barrier properties under normal and hypoxic conditions

Stroke is one of the leading causes of mortality and disability worldwide. The blood-brain barrier (BBB) plays an important role in maintaining brain homeostasis by tightly regulating the exchange of substances between circulating blood and brain parenchyma. BBB disruption is a common pathologic feature of stroke and traumatic brain injury. Understanding the cellular and molecular events that affect the BBB after ischaemic brain injury is important to improve patient prognosis.
We have previously shown that microRNA-212/132 is elevated in hypoxic brain microvascular endothelial cells and acts through suppressing the expression of direct microRNA-212/132 target genes with function at the BBB: claudin-1, junctional adhesion molecule 3 (Jam3) and tight-junction associated protein 1 (Tjap1). While the role of claudin-1 and Jam3 at the BBB is well known, the role of Tjap1 is still unclear. The aim of this work was therefore to characterize the role of Tjap1 in brain endothelial cells using a knock-down (KD) approach in established murine in vitro BBB models cEND and cerebEND. Tjap1 KD was established by stable transfection of a plasmid expressing shRNA against Tjap1. The successful downregulation of Tjap1 mRNA and protein was demonstrated by qPCR and Western blot. Tjap1 KD resulted in impaired barrier properties of endothelial cells as shown by lower TEER values and higher paracellular permeability. Interestingly, the Tjap1 KD cells showed lower cell viability and proliferation but migrated faster in a wound healing assay. In the tube formation assay, Tjap1 KD cell lines showed a lower angiogenic potential due to a significantly lower tube length and number as well as a lower amount of branching points in formed capillaries. Tjap1 KD cells showed changes in gene and protein expression. The TJ proteins claudin-5, Jam3 and ZO-1 were significantly increased in Tjap1 KD cell lines, while occludin was strongly decreased. In addition, efflux pump P-glycoprotein was downregulated in Tjap1 KD cells. Oxygen-glucose deprivation (OGD) is a method to mimic stroke in vitro. Brain endothelial cell lines treated with OGD showed lower barrier properties compared to cells cultured under normal condition. These effects were more severe in Tjap1 KD cells, indicating active Tjap1 involvement in the OGD response in brain microvascular endothelial cells.
We thus have shown that Tjap1 contributes to a tight barrier of the BBB, regulates cell viability and proliferation of endothelial cells, suppresses their migration and promotes new vessel formation. This means that Tjap1 function is important for mature BBB structure in health and disease.
Es handelt sich um eine experimentelle Arbeit zur Untersuchung der Auswirkungen verschiedener Volumensubstitutionslösungen auf die Integrität der Leber in der CLP-induzierten Sepsis der Ratte. 40 Ratten wurden in 5 Grp. eingeteilt, anästhesiert und median laparotomiert. Während das Coecum der Sham-Tiere im ursprünglichen Zustand verblieb, erhielten alle anderen Tiere eine CLP. Die Tiere wurden im Anschluss entpsprechend ihrer Gruppe entweder nur mit dem Grundbedarf an NaCl (Sham) oder mit dem Grundbedarf an NaCl und dem jeweiligen Substitutionsmittel NaCl , SteroIso, Gelafundin, 6%HES 130/0,4 infundiert. Der Versuch lief über 24 h. Danach wurde die Tiere reanästhesiert, laparotomiert und eine in-vivo-Mikroskopie der Leber durchgeführt. Im Anschluss wurden sowohl hämodynamische Werte, Serumparameter und Zytokinwerte als auch histopathologische Daten ermittelt.
Ziel dieser Arbeit war es, die antiinflammatorischen Effekte von Anthocyanen und Anthocyanidinen mit besonderem Augenmerk auf Delphinidin zu vergleichen. Desweiteren sollten im statistischen Vergleich Dosierungen gefunden werden, bei welchen die größte antiinflammatorische Wirksamkeit von Anthocyanen und Anthocyanidinen zu erwarten wäre und ein eventueller Unterschied in der Wirkung der verschiedenen Stoffe berechnet werden.
Die Literaturrecherche mittels der Datenbanken „Pubmed“ und „The Cochrane Library“ lieferte 24 Studien, die die Einschlusskriterien erfüllten. Für Delphinidin allein war die Datenlage zu diesem Zeitpunkt noch zu dürftig, weswegen die Suche auf die Begriffe „Anthocyanins“ in Kombination mit „Antiinflammatory Agents“ ausgedehnt wurde.
Die statistische Auswertung der Daten erfolgte unter der Fragestellung: „ Haben die Variablen Wirkstoff, Wirkdauer, Menge an Wirkstoff und Art (m-RNA oder Protein“ jeweils unter Kontrolle der anderen Variablen einen Einfluss auf die Reduktion von inflammatorischen Markern?“ Die wichtigsten Ergebnisse, die unter dieser Fragestellung errechnet werden konnten, lauten: 1. PDG zeigt eine bessere antiinflammatorische Wirksamkeit als DP und C3G, 2. Luteolin zeigt eine bessere antiinflammatorische Wirksamkeit als Cyanidin.
Gründe für eine höhere antiinflammatorische Wirksamkeit von PDG im Vergleich zu DP und C3G sind derzeit noch offen. Zu diskutieren ist unter anderem, ob DP und C3G biochemisch instabilere Komplexe sind und schneller zerfallen. Grund für eine höhere antiinflammatorische Wirksamkeit von Luteolin im Vergleich zu Cyanidin könnte eine Doppelbindung zwischen C2 und C3 sein, die bei Luteolin, nicht aber bei Cyanidin zu finden ist. In der Zusammenschau dieser Ergebnisse ist festzustellen, dass weitere Studien für eine möglichst genaue statistische Analyse nötig sind, vor allem in Bezug auf die antiinflammatorischen Effekte von Delphinidin.
Das Phänomen der ischämischen Präkonditionierung beschreibt einen endogenen Schutzmechanismus, der in einer erhöhten Toleranz des Gewebes gegenüber ischämischen Episoden resultiert. Volatile Anästhetika sind in der Lage, diesen Mechanismus zu aktivieren und somit betroffene Gewebe zu präkonditionieren. Die ischämische Präkonditionierung zeigt an Kaninchen ein biphasisches Verlaufsmuster, bestehend aus einem frühen ersten Fenster sowie einem späten zweiten Fenster der Präkonditionierung. Beide Fenster sind durch eine Phase ohne kardioprotektiven Effekt getrennt. Ziel der vorliegenden Arbeit war es, für das volatile Anästhetikum Desfluran ebenfalls dieses biphasische Zeitmuster nachzuweisen sowie die Rolle von Stickstoffmonoxid (NO) in diesem Prozess zu charakterisieren. Wir führten unsere Untersuchungen in einem in vivo-Herzinfarktmodell an Kaninchen durch. Wir konnten zeigen, dass Desfluran ein erstes Fenster der Präkonditionierung induziert, welches bis zu zwei Stunden nach Abflutung des volatilen Agens nachweisbar ist. Weiterhin induzierte Desfluran ein zweites Fenster der Präkonditionierung, dessen kardioprotektiver Effekt nach 24 Stunden einsetzt und bis zu 72 Stunden nach Applikation des Anästhetikums nachweisbar ist. Erstes und zweites Fenster der Präkonditionierung waren durch eine Episode ohne nachweisbaren kardioprotektiven Effekt getrennt. 96 Stunden nach Abflutung des Anästhetikums war keine präkonditionierende Wirkung mehr nachweisbar. Um die Rolle von NO beim zweiten Fenster der Desfluran-induzierten Präkonditionierung zu untersuchen, verabreichten wir den NO-Synthase-Blocker L-omega-Nitro-Arginin (LNA) vor der Koronararterienokklusion. Anhand unserer Ergebnisse konnten wir nachweisen, dass die Desfluran-induzierte Präkonditionierung des Kaninchenmyokards ein der ischämischen Präkonditionierung ähnliches charakteristisches biphasisches Verlaufsmuster aufweist und das endogen synthetisiertes NO als Mediator des zweiten Fensters der Desfluran-induzierten Präkonditionierung wirkt.
Untersuchungen zu immunologischen Pathomechanismen bei der Entstehung chronischer Schmerzsyndrome

Im Rahmen der vorliegenden Arbeit wurden anhand eines Tiermodells zunächst Hinweise für die pathogene Relevanz von Serumfaktoren für die Entstehung von chronischen Schmerzsyndromen und assoziierten Symptomen gesucht. Es zeigten sich bei Versuchsmäusen nach intraperitonealer Injektion von Serum eines Patienten mit komplexem regionalem Schmerzsyndrom (CRPS) im Vergleich zu Kontrolltieren, denen Serum von gesunden Blutspendern injiziert wurde, Veränderungen des Spezies-spezifischen, explorativen Verhaltensmusters, welche erste Hinweise auf schmerzassoziiertes Verhalten liefern. Aufgrund dieser Befunde und der Hypothese der Präsenz einer humoralen Autoimmunreaktion bei der Entstehung chronischer Schmerzsyndrome wurde die Seroprävalenz für Antikörper gegen eine Vielzahl potentieller Autoantigene bei Patienten mit CRPS oder Fibromyalgiesyndrom im Vergleich zu gesunden Kontrollprobanden mittels immunhistochemischer Färbungen gegen murines Gewebe untersucht, wobei kein für die beschriebenen Schmerzerkrankungen pathognomonischer Autoantikörper identifiziert werden konnte. Die vorliegende Arbeit erfüllte ihren Zweck als Pilotprojekt, indem sie wichtige Daten lieferte, die für weitere Untersuchungen wegweisend sind. Dies bezieht sich im Besondern auf notwendige Änderungen bei den In-vitro-Methoden zur Detektion von Autoantikörpern. Zudem müssen die In-vivo-Experimente unter Einbeziehung sensorischer Verhaltenstests wiederholt und durch Fraktionierung der injizierten Serumproben erweitert werden, um gegebenenfalls Autoantikörper als Pathogene der Erkrankungen identifizieren zu können. Die in der vorliegenden Arbeit beschriebenen Erkenntnisse bieten einen Einblick in mögliche pathophysiologische Mechanismen bei der Entstehung von chronischen, idiopathischen Schmerzzuständen und könnten als Basis für neue Überlegungen über den Einsatz immunmodulierender Therapiestrategien bei derartigen Syndromen dienen.
In dieser Studie wurde Prilocain, ein kurz- bis mittellangwirksames Lokalanästhetikum, mit einem der beiden langwirksamen Lokalanästhetika, Ropivacain bzw. Bupivacain kombiniert, mit dem Zweck ein neues Wirkprofil zu erhalten. Dazu wurden 120 Patienten, die sich einem handchirurgischen Eingriff in axillärer Plexusanästhesie unterzogen, randomisiert und doppelblind einer der vier Behandlungsgruppen zugeteilt: Gruppe PB erhielt 7 mg/kg KG Prilocain 1% + 0,75 mg/kg KG Bupivacain 0,5%, Gruppe PR 7 mg/kg KG Prilocain1% + 0,75 mg/kg KG Ropivacain 0,5%. Gruppe P erhielt 7 mg/kg KG Prilocain 1%, Gruppe R 3 mg/kg KG Ropivacain 0,5%. Entsprechend der Randomisierungsliste wurden die Lokalanästhetika unter kontinuierlicher Nervenstimulation in die Gefäß- Nervenscheide des Plexus brachialis eingebracht. Nach Plexusanlage wurden Sensibilität, Motorik und Temperaturempfinden in den 3 Hautarealen des N. medianus, radialis und ulnaris alle 5 Minuten bis zum Operationsbeginn, postoperativ alle 15 Minuten bis zur vollständigen Rückkehr aller Sinnesqualitäten überprüft. Der Zeitpunkt für das postoperative Auftreten von Schmerzen sowie der Zeitpunkt einer Schmerzmitteleinnahme wurden notiert. Die aktuelle Schmerzstärke wurde postoperativ alle 30 Minuten mittels einer Kategorialskala ermittelt. Die Austestung der Analgesie erfolgte sowohl prä- als auch postoperativ gesondert für jeden Hauptnerven mittels pin-prick Methode. Analog dazu wurde die motorische Blockade unter Berücksichtigung der jeweiligen Kennmuskeln überprüft. Zur statistischen Auswertung fand der Kruskal- Wallis Test und der U- Test von Mann und Withney Anwendung, nominale Variablen wurden mit dem Chi- Quadrat Test überprüft. Im Hinblick auf die Dauer der Anästhesie lagen die Werte der Gruppe R signifikant“ über denen der anderen 3 Gruppen (p<0,001). Bei der Prilocain- Gruppe ließ die Anästhesie gegenüber Gruppe PR (p=0,003) und Gruppe R (p<0,001) „signifikant“ früher nach. Im Vergleich mit Gruppe PB zeigte sich nur in den Hautarealen des N. medianus (p=0,005) und des N. ulnaris (p=0,002) ein „signifikantes“ Ergebnis, der N. radialis lieferte einen Trendwert (p=0,061). Gruppe PB und PR unterschieden sich in Bezug auf das N. ulnaris- Gebiet nicht (p=0,453), der N. radialis zeigte einen Trend (p=0,065). Das Gebiet des N. medianus blieb bei Gruppe PR „signifikant“ länger anästhesiert als bei Gruppe PB (p=0,033). Die Anästhesie ließ in allen 4 Gruppen zuerst im Gebiet des N. radialis nach, danach folgte der N. medianus, zuletzt der N. ulnaris. Die Analgesiedauer lag bei der Ropivacain- Gruppe in allen 3 Nervenarealen „signifikant“ über der von Gruppe PB, PR und P (p<0,001). Gruppe P erlangte ihr Diskriminierungsvermögen stattdessen „signifikant“ früher als die anderen Gruppen (p<0,001). Gruppe PR zeigte im Vergleich mit Gruppe PB in keinem der 3 Nervenareale einen signifikanten Unterschied. Bei der Ropivacain- Gruppe hielt die motorische Blockade „signifikant“ länger an als bei den anderen Gruppen (p<0,001), die Prilocain- Gruppe dagegen hatte „signifikant“ früher die volle Kontrolle über den Arm als Gruppe PB (p=0,009), PR (p<0,001), und R (p<0,001). Die Werte der Gruppe PB und PR waren nicht unterschiedlich. Bei der Rückkehr des Temperaturempfindens fanden sich dieselben Signifikanzen wie bei der Analgesiedauer und der Dauer der motorischen Blockade. Bei Gruppe R kehrte das Temperaturempfinden „signifikant“ später zurück als bei den anderen 3 Gruppen (p<0,001). Die Prilocain - Gruppe fühlte „signifikant“ vor Gruppe PB (p=0,01), PR (p<0,001) und R (p<0,001) Kälte. Zwischen Gruppe PB und PR fand sich auch hier kein Unterschied. Die Anzahl der Patienten, die ein Schmerzmittel einnahmen, war „nicht signifikant“ unterschiedlich zwischen den Gruppen. Die Dauer der Analgetikafreiheit zeigte nur zwischen Gruppe PB und R einen „signifikanten“ Unterschied (p=0,009), alle anderen Gruppen waren gleich. Es konnte gezeigt werden, dass eine Kombination der Lokalanästhetika Prilocain 1% und Ropivacain 0,5% bei axillärer Plexusblockade die Wirkungsdauer von Anästhesie, Analgesie und motorischer Blockade verlängert, ohne die Anschlagszeit zu verändern. Eine Kombination von Prilocain 1% und Bupivacain 0,5% verhielt sich bezüglich der Anschlagszeit und Wirkungsdauer äquipotent zur o.g. Lokalanästhetikakombination. Im Hinblick auf das geringere kardiotoxische Potential von Ropivacain gegenüber Bupivacain sollte aber der Kombination mit Ropivacain der Vorzug gegeben werden.
Temporary hypercapnia has been shown to increase cerebral blood flow (CBF) and might be used as a therapeutical tool in patients with severe subarachnoid hemorrhage (SAH). It was the aim of this study was to investigate the optimum duration of hypercapnia. This point is assumed to be the time at which buffer systems become active, cause an adaptation to changes of the arterial partial pressure of carbon dioxide (PaCO2) and annihilate a possible therapeutic effect. In this prospective interventional study in a neurosurgical ICU the arterial partial pressure of carbon dioxide (PaCO\(_2\)) was increased to a target range of 55 mmHg for 120 min by modification of the respiratory minute volume (RMV) one time a day between day 4 and 14 in 12 mechanically ventilated poor-grade SAH-patients. Arterial blood gases were measured every 15 min. CBF and brain tissue oxygen saturation (StiO\(_2\)) were the primary and secondary end points. Intracranial pressure (ICP) was controlled by an external ventricular drainage. Under continuous hypercapnia (PaCO\(_2\) of 53.17 ± 5.07), CBF was significantly elevated between 15 and 120 min after the start of hypercapnia. During the course of the trial intervention, cardiac output also increased significantly. To assess the direct effect of hypercapnia on brain perfusion, the increase of CBF was corrected by the parallel increase of cardiac output. The maximum direct CBF enhancing effect of hypercapnia of 32% was noted at 45 min after the start of hypercapnia. Thereafter, the CBF enhancing slowly declined. No relevant adverse effects were observed. CBF and StiO\(_2\) reproducibly increased by controlled hypercapnia in all patients. After 45 min, the curve of CBF enhancement showed an inflection point when corrected by cardiac output. It is concluded that 45 min might be the optimum duration for a therapeutic use and may provide an optimal balance between the benefits of hypercapnia and risks of a negative rebound effect after return to normal ventilation parameters.
In der Volumentherapie galt das Kolloid Hydroxyethylstärke (HES) lange Zeit als ideale Infusionslösung und war der in Deutschland am häufigsten eingesetzte Plasmaexpander. In den letzten Jahren mehrten sich jedoch die Hinweise, dass HES insbesondere bei kritisch Kranken zu einer akuten Nierenfunktionsverschlechterung beitragen könnte, welche das klinische Ergebnis wesentlich beeinflusst. Der genaue Pathomechanismus ist bis heute nicht geklärt. Bekannt ist, dass sich HES nach intravenöser Applikation in vielen verschiedenen Geweben ablagert, wobei eine renale Anreicherung bevorzugt in proximalen Tubuluszellen stattfindet. Histopathologisch finden sich große Mengen intrazellulärer Vesikel im Zytoplasma, welche zu einer Zellschwellung führen, die auch als osmotische Nephrose bezeichnet und als prinzipiell reversibel erachtet wird. Der zelluläre Abbau soll dabei im Allgemeinen über das lysosomale System stattfinden. Dieses ist fester Bestandteil der Autophagie, welche ein evolutionär in allen Eukaryonten konservierter stress-induzierter kataboler Prozess ist, der der zellulären Homöostase und energieeffizienten Selbstreinigung dient. Hierbei werden defekte Makromoleküle durch Lysosomen in ihre Grundbestandteile zerlegt und der Zelle als Bausteine wieder zur Verfügung gestellt.
In dieser Arbeit wurde in einem ersten Schritt der Einfluss von HES der 3. Generation auf die Viabilität von HK-2-Zellen mit zwei unabhängigen in vitro Assays überprüft. Diese beruhen auf einem Substratumsatz durch zytosolische bzw. mitochondriale Dehydrogenasen. Für beide Assays konnte zu verschiedenen Inkubationszeitpunkten bis 21 Stunden jeweils eine konzentrationsabhängige Viabilitätsreduktion durch HES festgestellt werden, welche auch nach einer „Regenerationsphase“ noch verringert nachweisbar und somit partiell reversibel war.
Im nächsten Schritt wurde die Hypothese überprüft, ob eine medikamentöse Induktion der Autophagie die beobachtete Viabilitätsreduktion abschwächen oder sogar aufheben kann. Hierbei wurde analog zu einer Arbeit von Liu et al. (2014) eine Inkubationszeit von insgesamt acht Stunden gewählt, da nach diesem Zeitraum eine perinukleäre Cluster-Bildung der Lysosomen beobachtet werden konnte, welche für eine erhöhte Autophagierate spricht. Es kamen im Folgenden HES-Lösungen von 0,75% zum Einsatz, da diese aufgrund einer Viabilitätsreduktion um zirka ein Drittel als am besten geeignet betrachtet wurden. Der Autophagieinduktor Everolimus zeigte hierbei in dem auf mitochondrialen Dehydrogenasen basierenden EZ4U-Assay eine fast vollständige Aufhebung der HES-vermittelten Viabilitätsreduktion. Dieser Effekt konnte durch den MAP-Kinase-Kinase-Blocker U0126 aufgehoben werden. Andere Autophagiemodulatoren hingegen bewirkten zumeist nur geringe Änderungen der Zellviabilität.
Zuletzt wurde auf Proteinebene untersucht, ob zentrale Moleküle bzw. Komplexe der Autophagie unter HES zum einen im zeitlichen Verlauf und zum anderen unter zusätzlichem Einfluss der Modulatoren eine Expressionsänderung aufwiesen. HES alleine bewirkte im zeitlichen Verlauf weitestgehend keine signifikante Expressionsänderung. Auch im Vergleich zu einer 0%-HES-Kontrolllösung konnten keine relevanten Unterschiede festgestellt werden. Die Koinkubation mit Everolimus führte zu einer erhöhten Expression der Quotienten ppERK/pERK und LC3BII/LC3BI, U0126 konnte dies jeweils weitestgehend wieder aufheben. Perifosin bewirkte ebenso wie 3-Methyladenin eine verringerte Expression von Akt, Chloroquin führte zu keiner signifikanten Expressionsänderung aller bestimmter Proteine. Darüber hinaus verursachten alle Modulatoren keine signifikante Expressionsänderung des zentralen Autophagiekomplexes Beclin1 sowie von LAMP2 und SQSTM1.
Insgesamt sprechen die Ergebnisse dieser Arbeit gegen eine allgemeine, direkte Beeinflussung von klassischer Autophagie durch HES. Der Autophagieinduktor Everolimus zeigte jedoch einen protektiven Effekt auf die Zellviabilität, welcher vermutlich über einen Autophagie-vermittelten Weg verursacht wird. Unsere Arbeitsgruppe konnte darüber hinaus eine HES-vermittelte Reduktion von ROS beobachten. Das Autophagienetzwerk ist eng mit der zellulären Redox-Homöostase verknüpft. Eine verminderte ROS-Bildung könnte zu einer verminderten Autophagierate und somit auch verringerten Zellviabilität führen, welche durch Everolimus kompensiert wird.
Bisher ungeklärt ist auch, auf welchem Weg HES in die Zelle aufgenommen wird. Denkbar wäre, dass größere HES-Moleküle via Endozytose in die Zelle gelangen. Hierbei ist der mTORC1-Komplex ein wichtiger Regulator, der durch Everolimus gehemmt wird und somit über eine verringerte HES-Aufnahme zu einer Viabilitätssteigerung führen könnte. Kleinere HES-Moleküle dagegen könnten über Glukose-Transporter aufgenommen werden, die möglicherweise über AMPK reguliert werden.
Ziel dieser vorliegenden Arbeit war es, die invasive Muskelbiopsie und deren Anteil an der definitiven Diagnosestellung von neuromuskulären Erkrankungen in einem selektionierten Patientengut zu untersuchen, um die Indikationsstellung für eine Muskelbiopsie zu überprüfen. Zu diesem Zweck wurden retrospektiv die Krankenakten, sowie die Patientendatenbank aus dem Maligne Hyperthermie-Labor der Klinik und Poliklinik für Anästhesiologie der Universität Würzburg ausgewertet. In die Auswertung wurden Patienten mit chronischer klinischer myopathischer Symptomatik, mit asymptomatisch persistierender CK-Erhöhung, mit Narkosezwischenfällen, und mit akuter klinischer Symptomatik (ohne Narkosezwischenfälle), eingeschlossen. An Diagnostik wurde neben einer körperlich-neurologischen Untersuchung, Laborkontrolle, EMG und Laktattest, bei all diesen Patienten eine Muskelbiopsie mit IVKT und histopathologischer Untersuchung durchgeführt. Bei 47% dieser 213 Patienten konnte durch die Muskelbiopsie mit histopathologischer/histochemischer Untersuchung und IVKT eine MH-Disposition oder anderweitige Myopathie diagnostiziert werden. Somit können in vielen Fällen Fragen der Vererblichkeit und des weiteren Krankheitsverlaufes der Myopathie für die weitere Lebensplanung des Patienten beantwortet werden
Patientencharakteristik und Einsatzlogistik beim Interhospitaltransport kritisch kranker Patienten

Der steigende Bedarf an Interhospitaltransporten, erforderte die Einführung einer zusätzlichen Transportoption für den arztbegleiteten Patiententransport in Bayern. Mit der Überarbeitung des Bayerischen Rettungsdienstgesetzes wurde die gesetzliche Grundlage für die Einführung und Tätigkeit des Verlegungsarztes (VA) in Bayern geschaffen. Dem voraus ging eine Analyse des Instituts für Notfallmedizin und Medizinmanagement München im Auftrag des Freistaats Bayern, die in verschiedenen Szenarien ermitteln sollte, wie der hohen Nachfrage an arztbegleiteten Verlegungen ohne die Einführung weiterer Intensivtransportmittel begegnet werden kann. Die vom Gesetzgeber beschlossene Variante war das Rendezvous-System aus VEF und RTW.
Ziel der vorliegenden Dissertation war es, den Intensivtransportwagen (ITW) und das Verlegungsarzteinsatzfahrzeug (VEF) hinsichtlich ihrer Einsatz- und Patientenstruktur zu analysieren und näher zu beschreiben. Des Weiteren sollte untersucht werden, ob sich durch die Anwendung der überarbeiteten Alarmierungsstrukturen Unterversorgungen und damit potenzielle Patientenschädigungen verhindern ließen.
Zu diesem Zweck, wurden retrospektiv mehr als 1500 Datensätze ausgewertet, die in einem Zeitraum von über zwei Jahren zur Qualitätssicherung prospektiv gesammelt wurden. Die beiden Transportvarianten (ITW oder VA+RTW) unterscheiden sich durch die personelle und materielle Ausstattung und sollten deshalb zielgerichtet zur Anwendung kommen.
Die Patienten-Fahrzeug-Zuteilung, wie sie durch den von Wurmb et al. konstruierten Algorithmus, sowie den neueingeführten Indikationskatalog erfolgte, fand in mehr als 90% der Fälle im Arzt-zu-Arzt-Gespräch Bestätigung, und ermöglichte dementsprechend eine zuverlässige, schnelle und gut anwendbare Einsatzdisposition. Fehldispositionen wurden sicher verhindert. ITW und VEF wurden meist zur lückenlosen Fortführung einer Intensivtherapie, und zu etwa gleichgroßen Anteilen für disponible und nicht-disponible Transporte eingesetzt. Auch wenn der Transport von Intensivpatienten mittels VA eine Ausnahme bleiben muss, so konnte doch gezeigt werden, dass eine Verlegung von invasiv beatmeten oder katecholaminpflichtigen Patienten nicht automatisch eine Unterversorgung zur Folge hat. Die Patienten des Intensivtransportwagens waren insgesamt schwerer erkrankt, deutlich häufiger invasiv beatmet oder katecholaminpflichtig als die VA-Patienten. Eine Patientengefährdung oder Schädigung konnte aus keinem der dokumentierten Fällen ermittelt werden.
Da sich beide Transportmittel im täglichen Routinebetrieb gut ergänzen, erscheint es sinnvoll an der dualen Struktur des arztbesetzten bodengebundenen Patiententransports festzuhalten.
Der Stellenwert des Interhospitaltransfers wird voraussichtlich auch in Zukunft weiter steigen, da die Bildung von Regionalen Behandlungsnetzwerken, sowie die zunehmende Spezialisierung von Fachkliniken einen erhöhten Austausch von Patienten zwischen unterschiedlichen Behandlungseinrichtungen erfordert. Aus diesem Grund wäre es notwendig in regelmäßigen Abständen die Situation des arztbegleiteten Patiententransports zu analysieren, sowohl um Optimierungsbedarf zu entdecken, als auch um Versorgungsengpässe, wie sie in der Vergangenheit zu verzeichnen waren, vorzubeugen. Mit den vorliegenden Ergebnissen stellt sich abschließend die Frage, ob das in Bayern erfolgreiche System, auch auf weitere Bundesländer übertragen werden könnte. Diese Arbeit könnte als Grundlage weiterer Untersuchungen dienen, um die Qualität und Effizienz des ärztlich begleiteten Patiententransports weiter zu verbessern.
Hintergrund
Im Rahmen der Pandemie des SARS-CoV-2-Virus erlangte das Patientenkollektiv der Schwangeren früh Aufmerksamkeit. Initial wurde angesichts sich früh abzeichnender Krankheitsfälle bei jüngeren Patienten mit einem erheblichen Aufkommen peripartal zu betreuender, COVID-19-positiver Schwangerer gerechnet.
Ziel der Arbeit
Diese Arbeit vermittelt einen Einblick in die SARS-CoV-2-Infektionszahlen im Rahmen der geburtshilflichen Anästhesie zu Beginn der Pandemie sowie während der zweiten Infektionswelle in Deutschland.
Methoden
Über das COALA-Register (COVID-19 related Obstetric Anaesthesia Longitudinal Assessment-Registry) wurden sowohl von März bis Mai 2020 als auch von Oktober 2020 bis Februar 2021 in Deutschland und der Schweiz wöchentlich prospektiv Daten zu Verdachts- und bestätigten SARS-CoV-2-Fällen bei Schwangeren zum Zeitpunkt der Geburt erhoben. Betrachtet wurden die Verteilung dieser auf die Anzahl der Geburten, Zentren und Erhebungswochen sowie mütterliche Charakteristika und Krankheitsverläufe.
Ergebnisse
Neun Zentren haben im Verlauf 44 SARS-CoV-2-positive Schwangere zum Zeitpunkt der Geburt bei 7167 Geburten (0,6 %) gemeldet (3 Fälle auf 2270 Geburten (0,4 %) und 41 Fälle auf 4897 Geburten (0,8 %)). Berichtet wurden 2 schwere COVID-19-Verläufe (n = 1 mit Todesfolge nach ECMO, n = 1 mit ECMO überlebt). Bei 28 (68 %) Patientinnen verlief die Infektion asymptomatisch. Ein Neugeborenes wurde im Verlauf positiv auf SARS-CoV‑2 getestet.
Schlussfolgerung
Mithilfe des Registers konnte das Auftreten von Fällen zu Beginn der Pandemie zeitnah eingeschätzt werden. Es traten sporadisch Verdachtsfälle bzw. bestätigte Fälle auf. Aufgrund fehlender flächendeckender Testung muss aber von einer Dunkelziffer asymptomatischer Fälle ausgegangen werden. Während der zweiten Infektionswelle wurden 68 % asymptomatische Fälle gemeldet. Jedoch kann es bei jungen, gesunden Patientinnen ohne das Vorliegen typischer Risikofaktoren zu schwerwiegenden Verläufen kommen.
(1) Background: Data on coronavirus 2 infection during pregnancy vary. We aimed to describe maternal characteristics and clinical presentation of SARS-CoV-2 positive women requiring intensive care treatment for COVID-19 during pregnancy and postpartum period based on data of a comprehensive German surveillance system in obstetric patients. (2) Methods: Data from COVID-19 Related Obstetric and Neonatal Outcome Study (CRONOS), a prospective multicenter registry for SARS-CoV-2 positive pregnant women, was analyzed with respect to ICU treatment. All women requiring intensive care treatment for COVID-19 were included and compared regarding maternal characteristics, course of disease, as well as maternal and neonatal outcomes. (3) Results: Of 2650 cases in CRONOS, 101 women (4%) had a documented ICU stay. Median maternal age was 33 (IQR, 30–36) years. COVID-19 was diagnosed at a median gestational age of 33 (IQR, 28–35) weeks. As the most invasive form of COVID-19 treatment interventions, patients received either continuous monitoring of vital signs without further treatment requirement (n = 6), insufflation of oxygen (n = 30), non-invasive ventilation (n = 22), invasive ventilation (n = 28), or escalation to extracorporeal membrane oxygenation (n = 15). No significant clinical differences were identified between patients receiving different forms of ventilatory support for COVID-19. Prevalence of preterm delivery was significantly higher in women receiving invasive respiratory treatments. Four women died of COVID-19 and six fetuses were stillborn. (4) Conclusions: Our cohort shows that progression of COVID-19 is rare in pregnant and postpartum women treated in the ICU. Preterm birth rate is high and COVID-19 requiring respiratory support increases the risk of poor maternal and neonatal outcome.
Mit dem Auftreten des SARS-CoV-2 Virus im Jahr 2020 war der Informationsgewinn für vulnerable Patientengruppen essentiell. Ziel dieser Arbeit war es maternale Charakteristika und das klinische Bild SARS-CoV-2 positiver Frauen mit Notwendigkeit einer intensivmedizinischen Behandlung während der Schwangerschaft und postpartal darzustellen, und diese Kohorte mit den SARS-CoV-2 positiven Schwangeren ohne intensivmedizinischen Handlungsbedarf zu vergleichen. Die Daten stammten aus dem deutschen CRONOS-Register, einem prospektiven, multizentrischen Register für SARS-CoV-2 positive schwangere Frauen. Eingeschlossen wurden alle schwangeren und postpartalen Frauen, die während ihrer SARS-CoV-2 Infektion auf eine ITS aufgenommen wurden. Diese wurden hinsichtlich maternaler Charakteristika, Krankheitsverlauf, sowie Outcomes verglichen.
In 101 von 2650 Fällen (4%) der Patientinnen des CRONOS-Registers, kam es zu einer Aufnahme auf die ITS. Als invasivste Form der COVID-19 Behandlung war bei 6 Patientinnen nur eine Überwachung notwendig, 30 Patientinnen benötigten eine Sauerstoffinsufflation, 22 wurden nicht-invasiv beatmet, 28 erhielten eine invasive Beatmung und bei 15 Frauen wurde die Behandlung zur ECMO-Therapie eskaliert. Es wurden keine klinisch signifikanten Unterschiede zwischen Patientinnen gefunden, die unterschiedliche Behandlungsformen benötigten. Die Gruppe der ITS und Non-ITS Patientinnen unterschied sich statistisch signifikant beim Einfluss von Alter, BMI bei Einschluss und der Herkunft. Die Prävalenz der Frühgeburtlichkeit war unter den invasiv behandelten Patientinnen signifikant höher und auch im Vergleich der ITS mit den Non-ITS Patientinnen zeigte sich ein signifikanter Unterschied. Vier Frauen verstarben an COVID-19 und sechs Feten der ITS-Gruppe waren Totgeburten.
Diese Kohorte zeigt, dass schwere COVID-19 Erkrankung bei schwangeren Frauen und Wöchnerinnen selten sind. Die Frühgeburtenrate ist hoch und COVID-19 mit Notwendigkeit einer Atemunterstützung erhöht das Risiko für ein schlechtes maternales und neonatales Outcome. Unter anderem ein höheres Alter und BMI sind mit einem höheren Risiko für eine ITS-Aufnahme verbunden.

We investigated the ionization profiles for open-ring (OR) and closed-ring (CR) forms of midazolam and drug-binding modes with heptakis-(2,3,6-tri-O-methyl)-β-cyclodextrin (trimethyl-β-cyclodextrin; TRIMEB) using molecular modeling techniques and quantum mechanics methods. The results indicated that the total net charges for different molecular forms of midazolam tend to be cationic for OR and neutral for CR at physiological pH levels. The thermodynamic calculations demonstrated that CR is less water-soluble than OR, mainly due to the maximal solvation energy (ΔG(CR)(solv = -9.98 kcal·mol ⁻¹), which has a minimal ΔG(OR)(solv) of -67.01 kcal·mol⁻¹. A cell viability assay did not detect any signs of TRIMEB and OR/CR-TRIMEB complex toxicity on the cEND cells after 24 h of incubation in either Dulbecco's Modified Eagles Medium or in heat-inactivated human serum. The molecular docking studies identified the more flexible OR form of midazolam as being a better binder to TRIMEB with the fluorophenyl ring introduced inside the amphiphilic cavity of the host molecule. The OR binding affinity was confirmed by a minimal Gibbs free energy of binding (ΔG(bind)) value of -5.57 ± 0.02 kcal·mol⁻¹, an equilibrium binding constant (K(b)) of 79.89 ± 2.706 μM, and a ligand efficiency index (LE(lig)) of -0.21 ± 0.001. Our current data suggest that in order to improve the clinical applications of midazolam via its complexation with trimethyl-β-cyclodextrin to increase drug's overall aqueous solubility, it is important to concern the different forms and ionization states of this anesthetic. Clinical trials of novel therapeutics for Alzheimer’s Disease (AD) have consumed a significant amount of time and resources with largely negative results. Repurposing drugs already approved by the Food and Drug Administration (FDA), European Medicines Agency (EMA), or Worldwide for another indication is a more rapid and less expensive option. Therefore, we apply the scaffold searching approach based on known amyloid-beta (Aβ) inhibitor tramiprosate to screen the DrugCentral database (n = 4,642) of clinically tested drugs. As a result, menadione bisulfite and camphotamide substances with protrombogenic and neurostimulation/cardioprotection effects were identified as promising Aβ inhibitors with an improved binding affinity (ΔGbind) and blood-brain barrier permeation (logBB). Finally, the data was also confirmed by molecular dynamics simulations using implicit solvation, in particular as Molecular Mechanics Generalized Born Surface Area (MM-GBSA) model. Overall, the proposed in silico pipeline can be implemented through the early stage rational drug design to nominate some lead candidates for AD, which will be further validated in vitro and in vivo, and, finally, in a clinical trial.
In this study, the ability of a multiwalled carbon nanotube functionalized with fluorescein isothiocyanate (MWCNT-FITC) was assessed as a prospective central nervous system-targeting drug delivery system to permeate the blood-brain barrier. The results indicated that the MWCNT-FITC conjugate is able to penetrate microvascular cerebral endothelial monolayers; its concentrations in the Transwell® system were fully equilibrated after 48 hours. Cell viability test, together with phase-contrast and fluorescence microscopies, did not detect any signs of MWCNT-FITC toxicity on the cerebral endothelial cells. These microscopic techniques also revealed presumably the intracellular localization of fluorescent MWCNT-FITCs apart from their massive nonfluorescent accumulation on the cellular surface due to nanotube lipophilic properties. In addition, the 1,000 ps molecular dynamics simulation in vacuo discovered the phenomenon of carbon nanotube aggregation driven by van der Waals forces via MWCN-TFITC rapid dissociation as an intermediate phase.
The purpose of this study was to develop and implement an in silico model of indigoid-based single-electron transistor (SET) nanodevices, which consist of indigoid molecules from natural dye weakly coupled to gold electrodes that function in a Coulomb blockade regime. The electronic properties of the indigoid molecules were investigated using the optimized density-functional theory (DFT) with a continuum model. Higher electron transport characteristics were determined for Tyrian purple, consistent with experimentally derived data. Overall, these results can be used to correctly predict and emphasize the electron transport functions of organic SETs, demonstrating their potential for sustainable nanoelectronics comprising the biodegradable and biocompatible materials.
The cytochrome P450 (CYP)3A4 enzyme affects the metabolism of most drug-like substances, and its inhibition may influence drug safety. Modulation of CYP3A4 by flavonoids, such as anthocyanins, has been shown to inhibit the mutagenic activity of mammalian cells. Considering the previous investigations addressing CYP3A4 inhibition by these substances, we studied the three-dimensional quantitative structure-activity relationship (3D-QSAR) in a series of anthocyanin derivatives as CYP3A4 inhibitors. For the training dataset (n=12), comparative molecular field analysis (CoMFA) and comparative molecular similarity index analysis (CoMSIA) yielded crossvalidated and non-crossvalidated models with a q (2) of 0.795 (0.687) and r (2) of 0.962 (0.948), respectively. The models were also validated by an external test set of four compounds with r (2) of 0.821 (CoMFA) and r (2) of 0.812 (CoMSIA). The binding affinity modes associated with experimentally derived IC50 (half maximal inhibitory concentration) values were confirmed by molecular docking into the CYP3A4 active site with r (2) of 0.66. The results obtained from this study are useful for a better understanding of the effects of anthocyanin derivatives on inhibition of carcinogen activation and cellular DNA damage.
The objective of the present investigation was to study the ability of sulfobutylether-\(\beta\)-cyclodextrin (SBECD) to form an inclusion complex with sevoflurane (SEV), a volatile anesthetic with poor water solubility. The inclusion complex was prepared, characterized and its cellular toxicity and blood-brain barrier (BBB) permeation potential of the formulated SEV have also been examined for the purpose of controlled drug delivery. The SEV-SBE\(\beta\)CD complex was nontoxic to the primary brain microvascular endothelial (pEND) cells at a clinically relevant concentration of sevoflurane. The inclusion complex exhibited significantly higher BBB permeation profiles as compared with the reference substance (propranolol) concerning calculated apparent permeability values (P\(_{app}\)). In addition, SEV binding affinity to SBE\(\beta\)CD was confirmed by a minimal Gibbs free energy of binding (ΔG\(_{bind}\)) value of -1.727 ± 0.042 kcal・mol\(^{-1}\) and an average binding constant (K\(_{b}\)) of 53.66 ± 9.24 mM indicating rapid drug liberation from the cyclodextrin amphiphilic cavity.
Diabetes mellitus is a common disease affecting more than 537 million adults worldwide. The microvascular complications that occur during the course of the disease are widespread and affect a variety of organ systems in the body. Diabetic retinopathy is one of the most common long-term complications, which include, amongst others, endothelial dysfunction, and thus, alterations in the blood-retinal barrier (BRB). This particularly restrictive physiological barrier is important for maintaining the neuroretina as a privileged site in the body by controlling the inflow and outflow of fluid, nutrients, metabolic end products, ions, and proteins. In addition, people with diabetic retinopathy (DR) have been shown to be at increased risk for systemic vascular complications, including subclinical and clinical stroke, coronary heart disease, heart failure, and nephropathy. DR is, therefore, considered an independent predictor of heart failure. In the present review, the effects of diabetes on the retina, heart, and kidneys are described. In addition, a putative common microRNA signature in diabetic retinopathy, nephropathy, and heart failure is discussed, which may be used in the future as a biomarker to better monitor disease progression. Finally, the use of miRNA, targeted neurotrophin delivery, and nanoparticles as novel therapeutic strategies is highlighted.
Alzheimer's disease (AD), the most common cause of dementia in the elderly, is a neurodegenerative disorder associated with neurovascular dysfunction and cognitive decline. While the deposition of amyloid β peptide (Aβ) and the formation of neurofibrillary tangles (NFTs) are the pathological hallmarks of AD-affected brains, the majority of cases exhibits a combination of comorbidities that ultimately lead to multi-organ failure. Of particular interest, it can be demonstrated that Aβ pathology is present in the hearts of patients with AD, while the formation of NFT in the auditory system can be detected much earlier than the onset of symptoms. Progressive hearing impairment may beget social isolation and accelerate cognitive decline and increase the risk of developing dementia. The current review discusses the concept of a brain–ear–heart axis by which Aβ and NFT inhibition could be achieved through targeted supplementation of neurotrophic factors to the cochlea and the brain. Such amyloid inhibition might also indirectly affect amyloid accumulation in the heart, thus reducing the risk of developing AD-associated amyloid cardiomyopathy and cardiovascular disease.
Retroviral vectors are potent tools for gene delivery and various biomedical applications. To accomplish a gene transfer task successfully, retroviral vectors must effectively transduce diverse cell cultures at different phases of a cell cycle. However, very promising retroviral vectors based on the foamy viral (FV) backbone lack the capacity to efficiently transduce quiescent cells. It is hypothesized that this phenomenon might be explained as the inability of foamy viruses to form a pre-integration complex (PIC) with nuclear import activity in growth-arrested cells, which is the characteristic for lentiviruses (HIV-1). In this process, the HIV-1 central polypurine tract (cPPT) serves as a primer for plus-strand synthesis to produce a “flap” element and is believed to be crucial for the subsequent double-stranded cDNA formation of all retroviral RNA genomes. In this study, the effects of the lentiviral cPPT element on the FV transduction potential in dividing and growth-arrested (G1/S phase) adenocarcinomic human alveolar basal epithelial (A549) cells are investigated by experimental and theoretical methods. The results indicated that the HIV-1 cPPT element in a foamy viral vector background will lead to a significant reduction of the FV transduction and viral titre in growth-arrested cells due to the absence of PICs with nuclear import activity.
The blood–brain barrier choline transporter (BBB-ChT) may have utility as a drug delivery vector to the central nervous system (CNS). We therefore initiated molecular docking studies with the AutoDock and AutoDock Vina (ADVina) algorithms to develop predictive models for compound screening and to identify structural features important for binding to this transporter. The binding energy predictions were highly correlated with r2=0.88, F=692.4, standard error of estimate =0.775, and P-value<0.0001 for selected BBB-ChT-active/inactive compounds (n=93). Both programs were able to cluster active (Gibbs free energy of binding <−6.0 kcal*mol-1) and inactive (Gibbs free energy of binding >−6.0 kcal*mol-1) molecules and dock them significantly better than at random with an area under the curve value of 0.86 and 0.84, respectively. In ranking smaller molecules with few torsional bonds, a size-related bias in scoring producing false-negative outcomes was detected. Finally, important blood–brain barrier parameters, such as the logBBpassive and logBBactive values, were assessed to predict compound transport to the CNS accurately. Knowledge gained from this study is useful to better understand the binding requirements in BBB-ChT, and until such time as its crystal structure becomes available, it may have significant utility in developing a highly predictive model for the rational design of drug-like compounds targeted to the brain.
P-glycoprotein (P-gp) is an ATP (adenosine triphosphate)-binding cassette transporter that causes multidrug resistance of various chemotherapeutic substances by active efflux from mammalian cells. P-gp plays a pivotal role in limiting drug absorption and distribution in different organs, including the intestines and brain. Thus, the prediction of P-gp–drug interactions is of vital importance in assessing drug pharmacokinetic and pharmacodynamic properties. To find the strongest P-gp blockers, we performed an in silico structure-based screening of P-gp inhibitor library (1,300 molecules) by the gradient optimization method, using polynomial empirical scoring (POLSCORE) functions. We report a strong correlation (r2=0.80, F=16.27, n=6, P<0.0157) of inhibition constants (Kiexp or pKiexp; experimental Ki or negative decimal logarithm of Kiexp) converted from experimental IC50 (half maximal inhibitory concentration) values with POLSCORE-predicted constants (KiPOLSCORE or pKiPOLSCORE), using a linear regression fitting technique. The hydrophobic interactions between P-gp and selected drug substances were detected as the main forces responsible for the inhibition effect. The results showed that this scoring technique might be useful in the virtual screening and filtering of databases of drug-like compounds at the early stage of drug development processes.
The dispersion routes of cyclodextrin complexes with nicardipine (NC), such as hydrophilic hydroxypropyl-\(\beta\)-cyclodextrin (NC/HP\(\beta\)CD) and hydrophobic triacetyl-\(\beta\)-cyclodextrin (NC/TA\(\beta\)CD), through the body for controlled drug delivery and sustained release have been examined. The two-compartment pharmacokinetic model described the mechanisms of how the human body handles with ingestion of NC-cyclodextrin complexes in gastrointestinal tract (GI), distribution in plasma, and their metabolism in the liver. The model showed that drug bioavailability was significantly improved after oral administration of cyclodextrin complexes. The mathematical significance of this study to predict nicardipine delivery using pharmacokinetic two-compartment mathematical model with linear ordinary differential equations (ODE) approach represents a valuable tool to emphasize its effectiveness and metabolizing rate and diminish the side effects.
Central nervous system dysfunction is an important cause of morbidity and mortality in patients with human immunodeficiency virus type 1 (HIV-1) infection and acquired immunodeficiency virus syndrome (AIDS). Patients with AIDS are usually affected by HIV-associated encephalitis (HIVE) with viral replication limited to cells of monocyte origin. To examine the molecular mechanisms underlying HIVE-induced dementia, the GSE4755 Affymetrix data were obtained from the Gene Expression Omnibus database and the differentially expressed genes (DEGs) between the samples from AIDS patients with and without apparent features of HIVE-induced dementia were identified. In addition, protein–protein interaction networks were constructed by mapping DEGs into protein–protein interaction data to identify the pathways that these DEGs are involved in. The results revealed that the expression of 1,528 DEGs is mainly involved in the immune response, regulation of cell proliferation, cellular response to inflammation, signal transduction, and viral replication cycle. Heat-shock protein alpha, class A member 1 (HSP90AA1), and fibronectin 1 were detected as hub nodes with degree values >130. In conclusion, the results indicate that HSP90A and fibronectin 1 play important roles in HIVE pathogenesis.
P-glycoprotein (P-gp)-mediated efflux system plays an important role to maintain chemical balance in mammalian cells for endogenous and exogenous chemical compounds. However, despite the extensive characterisation of P-gp potential interaction with drug-like molecules, the interaction of carbon nanoparticles with this type of protein molecule is poorly understood. Thus, carbon nanoparticles were analysed, such as buckminsterfullerenes (C20, C60, C70), capped armchair single-walled carbon nanotube (SWCNT or C168), and P-gp interactions using different molecular docking techniques, such as gradient optimisation algorithm (ADVina), Lamarckian genetic algorithm (FastDock), and shape-based approach (PatchDock) to estimate the binding affinities between these structures. The theoretical results represented in this work show that fullerenes might be P-gp binders because of low levels of Gibbs free energy of binding (ΔG) and potential of mean force (PMF) values. Furthermore, the SWCNT binding is energetically unfavourable, leading to a total decrease in binding affinity by elevation of the residual area (Ares), which also affects the π-π stacking mechanisms. Further, the obtained data could potentially call experimental studies using carbon nanostructures, such as SWCNT for development of drug delivery vehicles, to administer and assess drug-like chemical compounds to the target cells since organisms probably did not develop molecular sensing elements to detect these types of carbon molecules.
Background
Processing and analysis of DNA sequences obtained from next-generation sequencing (NGS) face some difficulties in terms of the correct prediction of DNA sequencing outcomes without the implementation of bioinformatics approaches. However, algorithms based on NGS perform inefficiently due to the generation of long DNA fragments, the difficulty of assembling them and the complexity of the used genomes. On the other hand, the Sanger DNA sequencing method is still considered to be the most reliable; it is a reliable choice for virtual modeling to build all possible consensus sequences from smaller DNA fragments.
Results
In silico and in vitro experiments were conducted: (1) to implement and test our novel sequencing algorithm, using the standard cloning vectors of different length and (2) to validate experimentally virtual shotgun sequencing using the PCR technique with the number of cycles from 1 to 9 for each reaction.
Conclusions
We applied a novel algorithm based on Sanger methodology to correctly predict and emphasize the performance of DNA sequencing techniques as well as in de novo DNA sequencing and its further application in synthetic biology. We demonstrate the statistical significance of our results.
Der Zusammenhang von neuropathischem Schmerz mit einer gestörten Blut-Nerven-
Schranke (BNS) ist bekannt. Die BNS wird durch Tight Junction Proteine (TJP) gebildet.
Netrin-1 (Ntn1) hat je nach Rezeptorbindung verschiedene Effekte auf TJP und somit auf
die Barriereeigenschaften.
In dieser Arbeit wurde im Tiermodell (Chronic Constriction Injury-CCI) untersucht, ob
Netrin-1 einen Einfluss auf die BNS hat und die Wirkung der Rezeptoren Unc5b und
Neogenin-1 beleuchtet. Es wurde untersucht, ob der barrierestabilisierende Netrin-1-
Spiegel auch von neuropathischen Schmerzen, im Speziellen durch „Chronic Regional
Pain Syndrom“ (CRPS), beeinflusst wird.
Männl. Wistar-Ratten wurde lokal Unc5b Antikörper injeziert oder nach Netrin-1 Gabe
der Neogeninrezeptor durch lokale Neogenin-1-siRNA Injektion geblockt. Die mRNA
Expression von Ntn1, seine Rezeptoren sowie der TJP (Claudine-Cldn) wurde mittels q-
PCR untersucht. Netrin-1 wurde im Rattennerven mittels Western Blot bestimmt. Die
Netrin-1-Spiegel im Plasma von CRPS Patient*innen und Kontrollen wurde mittels ELISA
bestimmt. Im Rattenmodell war die Ntn1 vermehrt exprimiert, die Proteinexpression
mittels Western Blot tendenziell vermindert. Die Claudinexpression war nach CCI
herabreguliert. Netrin-1-Injektion steigerte die Expression von Cldn5 und 19. Der
Netrin-1-Rezeptor UNC5B wird bei Neuropathie verstärkt und Neogenin-1 vermindert
exprimiert. Die Expression von Cldn 12 und Cldn19 war bei Blockade des Unc5b
Rezeptors gesteigert und bei Blockade des Neogenin-1 Rezeptors tendenziell
vermindert. Im Plasma von CRPS Patient*innen zeigte sich ein verminderter Netrin-1-
Spiegel.
Die Ergebnisse der vorliegenden Experimente legen nahe, dass Netrin-1 über die
Stabilisierung der Blut-Nerven-Schranke einen lindernden Effekt auf neuropathische
Schmerzen hat und sich auch die Expression dieses Proteins durch CRPS verändert.
Wir nahmen an, dass nach intramuskulärer Injektion von Koffein und Halothan in Schweinen mit Veranlagung (MHS) und ohne Verlangung (MHN) für eine maligne Hyperthermie, es zu einem dosisabhängigen intramuskulären Laktatanstieg kommt. Des weiteren ist die Ausdehnung des dadurch ausgelösten Hypermetabolismus nur auf ein kleines Areral um die Injektionsstelle begrenzt. Eine systemische MH-Krise wird nicht ausgelöst. Mikrodialysesonden wurden in die Hinterläufe von 7 MHN und 7 MHS Schweinen platziert und mit Ringerlösung perfundiert. Nach Einstellen eines Gleichgewichtes wurden Boli von Halothan und Koffein in aufsteigenden Konzentrationen in den Muskel appliziert. Für den zweiten Versuchsansatz, die regionale Ausbreitung betreffend, wurden wiederum Mikrodialysesonden im Abstand von 10 mm und 25 mm zum Injektionsort platziert. Das Laktat wurde photospektrometrisch im Dialysat gemessen. Durch intramuskuläre Halothan- und Koffeinapplikation kommt es zu einer dosisabhänigen Erhöhung der intramuskulären Laktatkonzentration mehr beim MHS als MHN-Tieren mit signifikantem Unterschied. Laktaterhöhungen fanden sich darüber hinaus nur an der Injektionsstelle der Trigger- substanzen, nicht jedoch in 10 und 25 mm Entfernung davon. Somit ist der Anstieg der lokalen Laktatkonzentration im Muskel nur auf ein kleines Areal um die Injektionsstelle begrenzt.
Visual Blood, a 3D animated computer model to optimize the interpretation of blood gas analysis

Acid–base homeostasis is crucial for all physiological processes in the body and is evaluated using arterial blood gas (ABG) analysis. Screens or printouts of ABG results require the interpretation of many textual elements and numbers, which may delay intuitive comprehension. To optimise the presentation of the results for the specific strengths of human perception, we developed Visual Blood, an animated virtual model of ABG results. In this study, we compared its performance with a conventional result printout. Seventy physicians from three European university hospitals participated in a computer-based simulation study. Initially, after an educational video, we tested the participants’ ability to assign individual Visual Blood visualisations to their corresponding ABG parameters. As the primary outcome, we tested caregivers’ ability to correctly diagnose simulated clinical ABG scenarios with Visual Blood or conventional ABG printouts. For user feedback, participants rated their agreement with statements at the end of the study. Physicians correctly assigned 90% of the individual Visual Blood visualisations. Regarding the primary outcome, the participants made the correct diagnosis 86% of the time when using Visual Blood, compared to 68% when using the conventional ABG printout. A mixed logistic regression model showed an odds ratio for correct diagnosis of 3.4 (95%CI 2.00–5.79, p < 0.001) and an odds ratio for perceived diagnostic confidence of 1.88 (95%CI 1.67–2.11, p < 0.001) in favour of Visual Blood. A linear mixed model showed a coefficient for perceived workload of −3.2 (95%CI −3.77 to −2.64) in favour of Visual Blood. Fifty-one of seventy (73%) participants agreed or strongly agreed that Visual Blood was easy to use, and fifty-five of seventy (79%) agreed that it was fun to use. In conclusion, Visual Blood improved physicians’ ability to diagnose ABG results. It also increased perceived diagnostic confidence and reduced perceived workload. This study adds to the growing body of research showing that decision-support tools developed around human cognitive abilities can streamline caregivers’ decision-making and may improve patient care.
Die Öffnung der Blut-Nerven-Schranke ist ein wichtiger Baustein in der Pathogenese neuropathischer Schmerzen. Die Blut-Nerven-Schranke schützt das periphere Nervensystem vor externen Einflüssen, wahrt die endoneurale Homöostase und trägt zur Aufrechterhaltung der neuronalen Signalweiterleitung bei. Sie wird durch die Pars epitheloidea des Perineuriums und endoneurale Gefäßzellen gebildet. Essentieller Bestandteil der Blut-Nerven-Schranke sind zwischen Perineural- und Gefäßzellen exprimierte Tight Junctions. Im Rahmen dieser Dissertation wurden die Tight Junction Proteine ZO-1, Claudin-1, -5, -19 und Occludin sowohl in einem Tiermodell neuropathischer Schmerzen, der Chronic Constriction Injury, als auch in Nervenbiopsien (N. suralis) von an Polyneuropathien erkrankten Patienten mittels Immunfluoreszenzfärbungen qualitativ und quantitativ untersucht.
Genetische und erworbene thrombophile Gerinnungsstörungen als Quelle chronischer Schmerzsyndrome

Anhand einer umfassenden Falldarstellung einer jungen Patientin mit einem lebensbedrohlichen Gesichtsschmerzsyndrom, das nach septischer Thrombose der periorbitalen venösen und arteriellen Gefäße aufgetreten war, wurde die Bedeutung einer medikamentösen Antikoagulation für die erfolgreiche Schmerztherapie herausgearbeitet. An diesem Fallbeispiel konnte aber auch gezeigt werden, dass keine sicheren Parameter für die Indikation einer solchen Gerinnungstherapie vorlagen. Die Bedeutung dieses Falls lag unzweifelhaft in der Erkenntnis, dass in einer anhaltenden Aktivierung des Kontaktsystems der Gerinnung ein bislang unterschätztes Potential für die Entstehung und Unterhaltung ungeklärter Schmerzen liegen könnte und nicht zuletzt auch daran, dass sich diese ätiologische Komponente in der Komplexität der Erkrankung diagnostisch nicht eindeutig sichern ließ. Mit der Translokation von LPS aus der intestinalen Mukosa in endothelial vorgeschädigte Gefäßabschnitte wurde eine Hypothese vorgetragen, die neben einer schwer detektierbaren inflammatorischen Komponente auch das prokoagulatorische Potential der Schmerzentstehung erklären könnte. Die prokoagulatorische Komponente dieses hypothetischen Entstehungs-mechanismus chronischer Schmerzen müsste, so die Arbeitshypothese, umso dominanter sein, wenn prokoagulatorisch wirksame genetische Faktoren bei den Patienten hinzukommen. Unter der Annahme, dass eine solche zusätzliche Diathese nicht nur eine Schrittmacherfunktion haben, sondern auch einen diagnostischen Beitrag liefern könnte, wurde dieses diagnostische Pilotprojekt mit der empirisch begründeten Heparintherapie von 97 Schmerzpatienten verbunden. Alle Pa-tienten wurden mit dem niedermolekularen Heparin Enoxaparin behandelt und nach zehn Behandlungstagen in vier verschiedene Respondergruppen (Gruppe 1 bis 4) eingeteilt. Diese Gruppen wurden auf fünf prothrombotische Parameter untersucht. Dazu wurden die Allelprävalenzen des Plasminogen Aktivator Inhibitor-(PAI-1 4G/5G) Polymorphismus, der Faktor V-Leiden-Mutation, der Prothrombin (G20210A) Genmutation sowie die Prävalenzen der Hyperfibrinogenämie und des Protein S-Mangels ermittelt. Mit Hilfe des exakten Fisher Tests wurden jeweils die Allelprävalenzen und Parameter sowohl der Respondergruppen 1 bis 3 mit einem Kollektiv der Allgemeinbevölkerung als auch mit dem Kollektiv der Non-Responder (Gruppe 4) verglichen. Die Prävalenz des Allels A der Faktor V-Leiden-Mutation G1691A war im Enoxaparin-Kollektiv bei den Respondern der Gruppen 1 bis 3 im Vergleich zur Allgemeinbevölkerung und zur Non-Respondergruppe (Gruppe 4) signifikant erhöht. Die Allelprävalenzen und Parameter der übrigen prokoagulatorischen Faktoren unterschieden sich von denen der Kontrollgruppen nicht. Anhand des Kallikrein-Kinin-Systems als möglichem Effektor des Hämosta-sesystems konnten Hinweise auf die kausale Wirksamkeit des nieder-molekularen Heparins Enoxaparin bei der Behandlung chronischer Schmerzen gegeben werden.
Emery-Dreifuss muscular dystrophy (EDMD) is a hereditary neuromuscular disorder characterized by slowly progressive muscle weakness, early contractures, and dilated cardiomyopathy. We reported an uneventful general anaesthesia using total intravenous anaesthesia (TIVA) for cardiac transplantation in a 19-year-old woman suffering from EDMD. In vitro contracture test results of two pectoralis major muscle bundles of the patient suggest that exposition to triggering agents does not induce a pathological sarcoplasmic calcium release in the lamin A/C phenotype. However, due to the lack of evidence in the literature, we would recommend TIVA for patients with EDMD if general anaesthesia is required.
Background
Malignant hyperthermia (MH), a metabolic myopathy triggered by volatile anesthetics and depolarizing muscle relaxants, is a potentially lethal complication of general anesthesia in susceptible patients. The implementation of modern inhalation anesthetics that research indicates as less potent trigger substances and the recommended limitations of succinylcholine use, suggests there may be considerable decline of fulminant MH cases. In the presented study, the authors analyzed suspected MH episodes during general anesthesia of patients that were referred to the Wuerzburg MH unit between 2007 and 2011, assuming that MH is still a relevant anesthetic problem in our days.
Methods
With approval of the local ethics committee data of patients that underwent muscle biopsy and in vitro contracture test (IVCT) between 2007 and 2011 were analyzed. Only patients with a history of suspected MH crisis were included in the study. The incidents were evaluated retrospectively using anesthetic documentation and medical records.
Results
Between 2007 and 2011 a total of 124 patients were tested. 19 of them were referred because of suspected MH events; 7 patients were diagnosed MH-susceptible, 4 MH-equivocal and 8 MH-non-susceptible by IVCT. In a majority of cases masseter spasm after succinylcholine had been the primary symptom. Cardiac arrhythmias and hypercapnia frequently occurred early in the course of events. Interestingly, dantrolene treatment was initiated in a few cases only.
Conclusions
MH is still an important anesthetic complication. Every anesthetist must be aware of this life-threatening syndrome at any time. The rapid onset of adequate therapy is crucial to avoid major harm and possibly lethal outcome. Dantrolene must be readily available wherever MH triggering agents are used for anesthesia.
Malignant hyperthermia is a rare but life-threatening complication of general anesthesia in predisposed patients usually triggered by potent inhalation anesthetics and/or the depolarizing muscle relaxant succinylcholine. The authors present a case of delayed sevoflurane-induced malignant hyperthermia in a 21-year-old male patient that was sufficiently treated by discontinuation of trigger agent application and dantrolene infusion. After surviving an MH episode diagnostic procedures are indicated to increase patient safety. In the presented case, the use of a novel minimal-invasive metabolic test with intramuscular injection of halothane and caffeine successfully confirmed MH susceptibility and hence might be an alternative for invasive in vitro contracture testing in selected cases.
Background
While the impact of volatile anaesthetics to induce malignant hyperthermia (MH) is abundantly clear, the role of succinylcholine still remains controversial. To evaluate the influence of succinylcholine on porcine MH events, the authors investigated the hemodynamic and metabolic responses in MH susceptible (MHS) and non-susceptible (MHN) swine following either succinylcholine or halothane application alone or a combination of both substances.
Methods
With approval of the local animal care committee 27 MHS and 30 MHN pigs were anaesthetized and mechanically ventilated. Fiberoptic probes for continuous PCO2 measurement were inserted into the femoral vein and the triceps muscle. Group A received succinylcholine 4 mg/kg, group B incremental doses of halothane (0.5, 1.0 vol%) and group C succinylcholine and halothane simultaneously. Vital signs were recorded continuously.
Results
Prior to drug application measured values did not differ between MHS and MHN. While MHN pigs did not show relevant alterations, succinylcholine, halothane and the combination of both lead to significant hemodynamic and metabolic changes in MHS swine.
Conclusions
Hemodynamic and metabolic alterations following succinylcholine were similar to halothane in MHS pigs. The combination of both pharmacological agents potentiated the observed effects. According to these results succinylcholine acted as an independent and supportive factor during onset of an MH episode.
Background:
The calcium sensitizer levosimendan is increasingly used to improve hemodynamics in patients with acutely decompensated heart failure. By binding to cardiac troponin C the conformation of the calcium-troponin C complex is stabilized, which leads to acceleration of actin-myosin crossbrigde formation and increased force generating capacity of muscle fibers. Besides indications in cardiac failure, beneficial effects of levosimendan in skeletal muscle disorders are currently evaluated. The aim of this study was to investigate differential effects of levosimendan on skeletal muscle of pigs with and without susceptibility to malignant hyperthermia (MH) in order to identify possible risks of this emerging drug for patients with predisposition to MH.
Methods:
Muscle bundles of 17 pigs (9 MH susceptible (MHS); 8 MH non-susceptible (MHN)) were excised under general anesthesia and examined in the tissue bath with increasing concentrations of levosimendan (0.065; 0.125; 0.5; 1.0; 10 and 50 μg/ml). Baseline tension and twitch force were monitored continuously. Data are presented as median and interquartile range. Statistical evaluation was performed using D’Agostino & Pearson test for normal distribution and student’s t test and 2-way ANOVA for differences between the groups. P < 0.05 was considered significant.
Results:
There were no differences between the groups concerning length, weight, initial twitch force and pre-drug resting tension of the investigated muscle strips. After an initial decrease in both groups, twitch amplitude was significantly higher in MHN (− 3.0 [− 5.2–0.2] mN) compared to MHS (− 7.5 [− 10.8- -4.5] mN) (p = 0.0034) muscle at an applied levosimendan concentration of 50 μg/ml. A marked increase in resting tension was detected following levosimendan incubation with 50 μg/ml in MHS muscle bundles (3.3 [0.9–6.1] mN) compared to MHN (− 0.7 [− 1.3–0.0] mN) (p < 0.0001).
Conclusions:
This in vitro investigation revealed the development of significant contractures in muscle bundles of MHS pigs after incubation with levosimendan. However, the effect appeared only at supra-therapeutic concentrations and further research is needed to determine the impact of levosimendan on MHS individuals in vivo.
Es wurden die Einsätze des ITW Würzburg des Jahres 2005 anhand der vorhandenen Intensivtransportprotokolle ausgewertet. Ziel der Arbeit war eine quantitative und qualitative Bestandsaufnahme des Interhospitalverkehrs unter besonderer Beachtung des Beatmungsmanagements. Insgesamt wurden 249 reine ITW-Einsätze im Jahr 2005 gefahren, von denen 243 durch ein vorhandenes Intensivtransportprotokoll dokumentiert waren. Die Dokumentation des Intensivtransportprotokolls war recht different. Zum einen hing die Qualität der Dokumentation vom transportbegleiteten Arzt ab und zum anderen wiesen einzelne Bereiche des Protokolls eine schlechtere Dokumentation auf als andere. Insgesamt waren Scores oder Angaben, die verschlüsselt einzutragen waren durchweg schlechter dokumentiert. Außerdem wurden viele Angaben im Übernahmestatus nicht dokumentiert, da diese oft schon im Verlaufsprotokoll protokolliert waren, was allerdings die digitale Erfassung erschwerte. Aufgrund dieser Beobachtungen entstand der Eindruck, dass hier ein Bedarf zur Vereinfachung des Protokolls besteht. Die Einsatzfrequenz war mit 0,68 Einsätzen pro Tag relativ gering, was mit dem großen Einsatzbereich und der geringen Einwohner-, und Krankenhausdichte zusammenhängt. Der ITW Würzburg wurde vorwiegend während der regulären Vorbehaltszeiten genutzt. Nur ein geringer Anteil (<10%) der Einsätze fanden außerhalb dieser Zeiten statt. Die Gesamteinsatzdauer im Median betrug 3 Std. 30 Min. pro ITW-Einsatz. Der längste Einsatzabschnitt war dabei die Nachbereitungszeit mit 1 Std. 15 Min. im Median. Sie beinhaltete sowohl die Wiederherstellung der Einsatzbereitschaft des Fahrzeugs, sowie die Rückfahrt zum Standort und Pausen. Der kürzeste Einsatzabschnitt war die Behandlungszeit vor Ort mit 30 Minuten im Median. Dabei fiel auf, dass zentripetale Transporte signifikant längere Behandlungszeiten vor Ort erforderten als zentrifugale Transporte. Nur 67,5% der Einsätze erreichten den Patienten innerhalb einer Stunde. Um eine Eintreffzeit von einer Stunde zu realisieren, wären mindestens zwei weitere ITW-Standorte in Schweinfurt und Aschaffenburg nötig. Dies erscheint aber vor dem Hintergrund der Auslastung des ITW Würzburgs, der geringen Zahl an Sofort-transporten und der Kosten nicht adäquat. Drei Viertel aller Transporte waren Ferntransporte, mit einer durchschnittlichen Entfernung der Zielklinik von 72 Km. Hier erscheint der Standort Würzburg hinsichtlich der guten Verkehrsanbindung als zentraler Standort des ITWs sinnvoll. Zentripetale und zentrifugale Transporte waren annähernd gleich vertreten. Das Patientenkollektiv des ITW Würzburg entsprach dem auf Intensivstationen. Knapp dreiviertel der Patienten war über 50 Jahre alt. Insgesamt überwog der Anteil der männlichen Patienten. Am häufigsten wurden Patienten mit intrakraniellen Blutungen, zerebralen Ischämien und Schädelhirntraumen transportiert. Dieser Gruppe folgten Patienten mit akutem Koronarsyndrom und Patienten mit Sepsis. Über die Hälfte, der mit dem ITW Würzburg transportierten Patienten, waren beatmungspflichtig. Dabei fiel auf, dass unabhängig von der Transportart, die Zahl der BIPAP beatmeten Patienten zunahm. Am stärksten war dieser Anstieg bei den zentrifugalen Transporten. Der PEEP wurde bei zentripetalen Transporten signifikant öfters erhöht als bei zentrifugalen oder zentri-zentralen Transporten, was wie schon die Behandlungszeit vor Ort, auf eine intensivere Therapie schließen lässt. Die Strukturqualität des ITW Würzburgs in Form von Ausstattung und Personal wird den geforderten Standards gerecht. Gerade vor dem Hintergrund des Patientenkollektivs kommt der Ausstattung des ITWs und der Qualifikation des Personals eine entscheidende Bedeutung zu. Bestehende Standards dürfen im Hinblick auf eine adäquate Versorgung der Patienten nicht unterschritten werden. Verbesserungsbedarf besteht lediglich hinsichtlich der Prozessqualität. Dies gilt insbesondere für die Dokumentation, die entsprechenden Dokumentationswerkzeuge und die Auslastung des ITW Würzburgs. Hinsichtlich der Dokumentation ist eine möglichst zeitnahe elektronische Doku-mentation wünschenswert mit Ausbau der im Rahmen dieser Arbeit erstellten Datenbank. Was die Auslastung des ITW betrifft, so ist sicherlich noch Potential vorhanden, das es auszuschöpfen gilt. Hier ist eine bessere Informationspolitik über die Vorteile und Einsatzmöglichkeiten des ITWs von Nöten. Das Beatmungsmanagement zeigte eine Intensivierung der Beatmung vor allem bei zentripetalen Transporten, was die Bedeutung des ITWs als Teil der Intensiv-medizinischen Versorgung unterstreicht.
At any moment in time, cells coordinate and balance their calcium ion (Ca\(^{2+}\)) fluxes. The term ‘Ca\(^{2+}\) homeostasis’ suggests that balancing resting Ca2+ levels is a rather static process. However, direct ER Ca\(^{2+}\) imaging shows that resting Ca\(^{2+}\) levels are maintained by surprisingly dynamic Ca\(^{2+}\) fluxes between the ER Ca\(^{2+}\) store, the cytosol, and the extracellular space. The data show that the ER Ca\(^{2+}\) leak, continuously fed by the high-energy consuming SERCA, is a fundamental driver of resting Ca\(^{2+}\) dynamics. Based on simplistic Ca\(^{2+}\) toolkit models, we discuss how the ER Ca\(^{2+}\) leak could contribute to evolutionarily conserved Ca\(^{2+}\) phenomena such as Ca\(^{2+}\) entry, ER Ca\(^{2+}\) release, and Ca\(^{2+}\) oscillations.
Therapeutic drug monitoring (TDM) is increasingly relevant for an individualized antibiotic therapy and subsequently a necessary tool to reduce multidrug-resistant pathogens, especially in light of diminishing antimicrobial capabilities. Critical illness is associated with profound pharmacokinetic and pharmacodynamic alterations, which challenge dose finding and the application of particularly hydrophilic drugs such as β-lactam antibiotics. Methods: Implementation strategy, potential benefit, and practicability of the developed standard operating procedures were retrospectively analyzed from January to December 2020. Furthermore, the efficacy of the proposed dosing target of piperacillin in critically ill patients was evaluated. Results: In total, 160 patients received piperacillin/tazobactam therapy and were subsequently included in the study. Of them, 114 patients received piperacillin/tazobactam by continuous infusion and had at least one measurement of piperacillin serum level according to the standard operating procedure. In total, 271 measurements were performed with an average level of 79.0 ± 46.0 mg/L. Seventy-one piperacillin levels exceeded 100 mg/L and six levels were lower than 22.5 mg/L. The high-level and the low-level group differed significantly in infection laboratory parameters (CRP (mg/dL) 20.18 ± 11.71 vs. 5.75 ± 5.33) and renal function [glomerular filtration rate (mL/min/1.75 m2) 40.85 ± 26.74 vs. 120.50 ± 70.48]. Conclusions: Piperacillin levels are unpredictable in critically ill patients. TDM during piperacillin/tazobactam therapy is highly recommended for all patients. Although our implementation strategy was effective, further strategies implemented into the daily clinical workflow might support the health care staff and increase the clinicians' alertness.
PURPOSE
The threat of national and international terrorism remains high. Preparation is the key requirement for the resilience of hospitals and out-of-hospital rescue forces. The scientific evidence for defining medical and tactical strategies often feeds on the analysis of real incidents and the lessons learned derived from them. This systematic review of the literature aims to identify and systematically report lessons learned from terrorist attacks since 2001.
METHODS
PubMed was used as a database using predefined search strategies and eligibility criteria. All countries that are part of the Organization for Economic Cooperation and Development (OECD) were included. The time frame was set between 2001 and 2018.
RESULTS
Finally 68 articles were included in the review. From these, 616 lessons learned were extracted and summarized into 15 categories. The data shows that despite the difference in attacks, countries, and casualties involved, many of the lessons learned are similar. We also found that the pattern of lessons learned is repeated continuously over the time period studied.
CONCLUSIONS
The lessons from terrorist attacks since 2001 follow a certain pattern and remained constant over time. Therefore, it seems to be more accurate to talk about lessons identified rather than lessons learned. To save as many victims as possible, protect rescue forces from harm, and to prepare hospitals at the best possible level it is important to implement the lessons identified in training and preparation.
Purpose: The threat of national and international terrorism remains high. Preparation is the key requirement for the resilience of hospitals and out-of-hospital rescue forces. The scientific evidence for defining medical and tactical strategies often feeds on the analysis of real incidents and the lessons learned derived from them. This systematic review of the literature aims to identify and systematically report lessons learned from terrorist attacks since 2001.
Methods: PubMed was used as a database using predefined search strategies and eligibility criteria. All countries that are part of the Organization for Economic Cooperation and Development (OECD) were included. The time frame was set between 2001 and 2018.
Results: 68 articles were included in the review. From these, 616 lessons learned were extracted and summarized into 15 categories. Die Präkonditionierung ist ein endogener Schutzmechanismus, bei dem die Toleranz einer Zelle gegen die Auswirkungen eines späteren ischämischen Schadens erhöht wird. Volatile Anästhetika sind in der Lage den durch die Ischämie verursachten Gewebsschaden zu vermindern, indem sie diesen Schutzmechanismus aktivieren. Ziel der vorliegenden Arbeit war die Untersuchung der CaMK II in der Anästhetika-induzierten Präkonditionierung und in der durch Metoprolol vermittelten Kardioprotektion, sowie der dosisabhängige Effekt von Metoprolol auf die Kardioprotektion und die Desfluran-induzierte Präkonditionierung. Dazu wurde der spezifische Inhibitor der CaMK II, KN-93, sowie der kardioselektive Betablocker Metoprolol in verschiedenen Dosierungen, jeweils vor der Koronarokklusion alleine oder zusammen mit Desfluran, verabreicht. Die Versuche wurden in einem in vivo Herzinfarktmodell an weißen Neuseelandkaninchen durchgeführt. Die Resultate dieser Untersuchungen ergaben, dass KN-93 die Infarktgröße nicht reduzierte und die Desfluran-induzierte Präkonditionierung aufgehoben wurde. Die effektive Hemmung der CaMK II wurde durch Western blot Analysen bestätigt. Die weiteren Ergebnisse ergaben, dass erst eine Metoprolol-Dosis von 1,75 mg/kg und 2,5 mg/kg die Myokardinfarktgröße signifikant verminderte. Die Western blot Analyse zeigte eine effektive Blockade beider Phosphorylierungsstellen des PLB. Außerdem wurde die Desfluran-induzierte Präkonditionierung durch 0,2 mg/kg Metoprolol abgeschwächt, durch 1,0 mg/kg, 1,75 mg/kg oder 2,5 mg/kg Metoprolol wurde sie vollständig aufgehoben. Aus diesen Ergebnissen resultiert, dass die Desfluran-induzierte Präkonditionierung über die Calcium/Calmodulin Kinase II abhängige Phosphorylierung des Phospholamban vermittelt wird. Dagegen wird die Metoprolol-induzierte Kardioprotektion während der Reperfusion durch die Blockade der PKA- und CaMK II-abhängigen Phosphorylierung des PLB vermittelt. Außerdem deuten die Ergebnisse auf eine negative Interaktion zwischen der Desfluran-induzierten Präkonditionierung und der beta-adrenergen Blockade hin.
Ziel dieser Arbeit ist es, anhand dreier quantitativer, systematischer Übersichtsarbeiten (Cochrane Reviews) die Möglichkeiten und Grenzen der EBM zu beleuchten. Dabei wird exemplarisch die Verwendung von hyperbarem Sauerstoff zur Therapie von chronischen Wunden, eines akuten ischämischen Schlaganfalls, sowie von Migräne- und Clusterkopfschmerzen untersucht.
Bei 155 Ärzten und Arzthelferinnen wurde ein Notfalltraining durchgeführt. Davor und danach wurde ein multiple-choice-Test mit acht Fragen zur Reanimation durchgeführt. Es zeigte sich ein signifikanter Anstieg der richtigen Antworten von 69,7 % auf 94,7%. Teilnehmer, deren letztes Training bis zu zwei Jahre zurücklag, erreichten signifikant bessere Ergebnisse. Ebenso wurde durch Fragebögen die Ausstattung von 118 Arztpraxen untersucht. Hier zeigten sich erhebliche Unterschiede je nach Fachrichtung des Arztes. Nur ca. 20 % der Ärzte sind mit modernen AED-Geräten zur Defibrillation ausgestattet. Gerätschaften zur Intubation halten ca. 90 % vor, Sauerstoff dagegen nur ca. 80%, Sauerstoff-Reservoire weniger als die Hälfte. Opiate sind nur bei 72% bevorratet, Cordarex nur bei einem Drittel. Folgende Forderung lassen sich ableiten: -verpflichtende Fortbildungsmaßnahmen zur Reanimation -dies mindestens alle zwei Jahre (im Rahmen des Qualitätsmanagements) -Verbesserung der notfallmedizinischen Geräte- und Medikamentenausstattung
Purpose
Anaemia is common in patients presenting with aneurysmal subarachnoid (aSAH) and intracerebral haemorrhage (ICH). In surgical patients, anaemia was identified as an idenpendent risk factor for postoperative mortality, prolonged hospital length of stay (LOS) and increased risk of red blood cell (RBC) transfusion. This multicentre cohort observation study describes the incidence and effects of preoperative anaemia in this critical patient collective for a 10-year period.
Methods
This multicentre observational study included adult in-hospital surgical patients diagnosed with aSAH or ICH of 21 German hospitals (discharged from 1 January 2010 to 30 September 2020). Descriptive, univariate and multivariate analyses were performed to investigate the incidence and association of preoperative anaemia with RBC transfusion, in-hospital mortality and postoperative complications in patients with aSAH and ICH.
Results
A total of n = 9081 patients were analysed (aSAH n = 5008; ICH n = 4073). Preoperative anaemia was present at 28.3% in aSAH and 40.9% in ICH. RBC transfusion rates were 29.9% in aSAH and 29.3% in ICH. Multivariate analysis revealed that preoperative anaemia is associated with a higher risk for RBC transfusion (OR = 3.25 in aSAH, OR = 4.16 in ICH, p < 0.001), for in-hospital mortality (OR = 1.48 in aSAH, OR = 1.53 in ICH, p < 0.001) and for several postoperative complications.
Conclusions
Preoperative anaemia is associated with increased RBC transfusion rates, in-hospital mortality and postoperative complications in patients with aSAH and ICH.
Der unbehandelte Herz-Kreislauf-Stillstand führt zwangsläufig zum Tod des Patienten. Die anfängliche Überlebensrate von 90% fällt mit jeder weiteren Minute um 10%. Von neurologischen Schäden ist ab der 4. Minute eines Herz-Kreislauf-Stillstands auszugehen. Nach 10 Minuten kann man schon nicht mehr von einer erfolgreichen Reanimation ausgehen. Die vorliegende Untersuchung beschäftigt sich mit dem Problem, das sich dem First-Responder bei Eintreffen am Einsatzort stellt. Um die Chancen auf eine langfristig erfolgreiche Reanimation des Patienten zu optimieren, ist es wichtig, dass der einzelne First-Responder in der Lage ist, den Zeitvorteil von 5 Minuten vor Eintreffen weiterer professioneller Hilfe mit einer hochwertigen und vor allem gleichmäßigen Reanimation zu überbrücken. In der vorliegenden Arbeit wurde untersucht, inwieweit der First-Responder hierbei durch den Einsatz eines halbautomatischen Hilfsgeräts unterstützt wird.
Der perioperative Myokardinfarkt ist eine der Hauptursachen perioperativer Morbidität und Mortalität, wobei bis zu 40% der Patienten an einem akuten Ereignis versterben. Eine der Hauptaufgaben der anästhesiologischen Forschung besteht daher in der Entwicklung neuer, verbesserter Strategien sowohl in der Erkennung und Prophylaxe als auch in der Therapie perioperativer Myokardischämien. In einer wegweisenden Studie zeigten Kersten et al. 1997 erstmals einen kardioprotektiven Effekt für volatile Anästhetika: Die Verabreichung von Isofluran vor der Induktion eines Ischämie-Reperfusions-Schadens (I/R-Schaden) führte zu einer signifikanten Reduktion der resultierenden Infarktgröße. Diese kardioprotektiven Eigenschaften wurden in der Folge auch für andere volatile Anästhetika aufgezeigt, ebenso wie die Induktion eines zweiten Fensters der Präkonditionierung (SWOP) beginnend 24h post-donum. Essentielle molekulare Mechanismen der protektiven Signalkaskade sind hierbei u.a. eine gesteigerte Synthese von Prostaglandinen sowie die erhöhte Produktion von Stickstoffmonoxid (NO). Vergleichbare NO-abhängige, kardioprotektive Wirkungen zeigen sich ebenso für Agonisten der Peroxysomen-Proliferator-aktivierten Rezeptoren (PPAR) α und γ. Die PPAR sind Transkriptionsfaktoren, die nach Liganden-vermittelter Aktivierung (u.a. 15d-Prostaglandin J2, 15d-PGJ2) ihre Wirkungen entfalten. Daher testeten wir die Hypothese, dass das zweite Fenster der Desfluran-induzierten Präkonditionierung durch eine erhöhte Produktion von 15d-PGJ2 mit der nachfolgenden Aktivierung von PPAR α und γ vermittelt wird. Die Experimente wurden in einem etablierten in-vivo Herzinfarktmodell des Kaninchens durchgeführt. Männliche weiße Neuseeland-Kaninchen wurden randomisiert 6 Gruppen zugeteilt. Alle Tiere erhielten eine 30-minütige Okklusion der linken Koronararterie (KAO) gefolgt von einer 3-stündigen Reperfusion. Zwei Gruppen erhielten entweder Desfluran (1.0 MAC, DES) oder Raumluft (Kontrolle, KON) 24h vor KAO. In weiteren Gruppen erhielten die Tiere den PPARα-Antagonisten GW6471 oder den PPARγ-Antagonisten GW9662, jeweils alleine (GW6471 bzw. GW9662) oder in Kombination mit vorheriger Desfluranapplikation (DES+GW6471 bzw. DES+GW9662). Die Infarktgröße (IS/AAR) wurde nach Bestimmung des Ischämieareals (AAR) und des Infarktareals (IS) gravimetrisch nach TTC-Färbung ermittelt. Sechs identische Gruppen wurden instrumentiert zur Entnahme des Herzens und anschließender Analyse der PPAR-DNA-Bindungsaktivität sowie der Gewebekonzentrationen von 15d-PGJ2 und NO mittels spezifischer Assays. Als Ergebnis zeigte sich eine signifikant reduzierte Herzinfarktgröße (DES 42,2±3,1%* vs. KON 61,8±2,8%; *p<0,05) 24h nach der Applikation des volatilen Anästhetikums. Dieser kardioprotektive Phänotyp war begleitet von einer erhöhten PPAR-DNA-Bindungsaktivität (DES 289,9±33RLU* vs. KON 102,9±18RLU, *p<0,05), als auch erhöhten Gewebskonzentrationen von 15d-PGJ2 (DES 224,4±10,2pg/ml* vs. KON 116,9±14,2pg/ml) und NO (DES 14,9±0,7μM* vs. KON 5,4±0,7μM). Unter spezifischer pharmakologischer Blockade von PPARα oder PPARγ kam es zum Verlust dieser protektiven Effekte, wobei sowohl die Infarktgrößenreduktion (DES+GW6471 60,0±2,8% bzw. DES+GW9662 56,5±2,9%), als auch die Elevation der PPAR-DNA-Bindungsaktivität (DES+GW6471 128,1±20,9RLU bzw. DES+GW9662 91,2±31RLU) und der myokardialen NO Konzentrationen (DES+GW6471 5,7±0,3μM bzw. DES+GW9662 5,9±0,9μM) aufgehoben wurde. Unbeeinflusst von der Blockade der PPAR blieb der durch Desfluran erhöhte 15d-PGJ2-Spiegel (DES+GW6471 199,5±8,1pg/ml* und DES+GW9662 179,0±11,7pg/ml*; p<0,05 vs. KON). Insgesamt zeigen die Ergebnisse dieser Arbeit, dass das zweite Fenster der Desfluran-induzierten Präkonditionierung durch die Aktivierung der PPARα und γ vermittelt wird. Die Applikation von Desfluran scheint hierbei durch die erhöhte Produktion von 15d-PGJ2 eine Signalkaskade in Gang zu setzen, welche nicht nur in der Aktivierung von PPARα und PPARγ resultiert, sondern ebenso durch die konsekutiv erhöhte myokardiale Verfügbarkeit von NO die Kardioprotektion vermittelt.
Background
Acute respiratory distress syndrome (ARDS) is a complex clinical diagnosis with various possible etiologies. One common feature, however, is pulmonary permeability edema, which leads to an increased alveolar diffusion pathway and, subsequently, impaired oxygenation and decarboxylation. A novel inhaled peptide agent (AP301, solnatide) was shown to markedly reduce pulmonary edema in animal models of ARDS and to be safe to administer to healthy humans in a Phase I clinical trial. Here, we present the protocol for a Phase IIB clinical trial investigating the safety and possible future efficacy endpoints in ARDS patients.
Methods
This is a randomized, placebo-controlled, double-blind intervention study. Patients with moderate to severe ARDS in need of mechanical ventilation will be randomized to parallel groups receiving escalating doses of solnatide or placebo, respectively. Before advancing to a higher dose, a data safety monitoring board will investigate the data from previous patients for any indication of patient safety violations. The intervention (application of the investigational drug) takes places twice daily over the course of 7 days, ensued by a follow-up period of another 21 days.
Discussion
The patients to be included in this trial will be severely sick and in need of mechanical ventilation. The amount of data to be collected upon screening and during the course of the intervention phase is substantial and the potential timeframe for inclusion of any given patient is short. However, when prepared properly, adherence to this protocol will make for the acquisition of reliable data. Particular diligence needs to be exercised with respect to informed consent, because eligible patients will most likely be comatose and/or deeply sedated at the time of inclusion.

Background: Acute respiratory failure is the most important organ dysfunction of COVID-19 patients. While non-invasive ventilation (NIV) and high-flow nasal cannula (HFNC) oxygen are frequently used, efficacy and safety remain uncertain. Benefits and harms of awake prone positioning (APP) in COVID-19 patients are unknown. Methods: We searched for randomized controlled trials (RCTs) comparing HFNC vs. NIV and APP vs. standard care. We meta-analyzed data for mortality, intubation rate, and safety. Results: Five RCTs (2182 patients) were identified. While it remains uncertain whether HFNC compared to NIV alters mortality (RR: 0.92, 95% CI 0.65–1.33), HFNC may increase rate of intubation or death (composite endpoint; RR 1.22, 1.03–1.45). We do not know if HFNC alters risk for harm. APP compared to standard care probably decreases intubation rate (RR 0.83, 0.71–0.96) but may have little or no effect on mortality (RR: 1.08, 0.51–2.31). Conclusions: Certainty of evidence is moderate to very low. There is no compelling evidence for either HFNC or NIV, but both carry substantial risk for harm. The use of APP probably has benefits although mortality appears unaffected.
Background
The viral load and tissue distribution of severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) remain important questions. The current study investigated SARS-CoV-2 viral load, biodistribution and anti-SARS-CoV-2 antibody formation in patients suffering from severe corona virus disease 2019 (COVID-19) induced acute respiratory distress syndrome (ARDS).
Methods
This is a retrospective single-center study in 23 patients with COVID-19-induced ARDS. Data were collected within routine intensive care. SARS-CoV-2 viral load was assessed via reverse transcription quantitative polymerase chain reaction (RT-qPCR). Overall, 478 virology samples were taken. Anti-SARS-CoV-2-Spike-receptor binding domain (RBD) antibody detection of blood samples was performed with an enzyme-linked immunosorbent assay.
Results
Most patients (91%) suffered from severe ARDS during ICU treatment with a 30-day mortality of 30%. None of the patients received antiviral treatment. Tracheal aspirates tested positive for SARS-CoV-2 in 100% of the cases, oropharyngeal swabs only in 77%. Blood samples were positive in 26% of the patients. No difference of viral load was found in tracheal or blood samples with regard to 30-day survival or disease severity. SARS-CoV-2 was never found in dialysate. Serologic testing revealed significantly lower concentrations of SARS-CoV-2 neutralizing IgM and IgA antibodies in survivors compared to non-survivors (p = 0.009).
Conclusions
COVID-19 induced ARDS is accompanied by a high viral load of SARS-CoV-2 in tracheal aspirates, which remained detectable in the majority throughout intensive care treatment. Remarkably, SARS-CoV-2 RNA was never detected in dialysate even in patients with RNAemia. Viral load or the buildup of neutralizing antibodies was not associated with 30-day survival or disease severity.
Background
Evidence concerning combined general anesthesia (GA) and thoracic epidural analgesia (EA) is controversial and the procedure appears heterogeneous in clinical implementation. We aimed to gain an overview of different approaches and to unveil a suspected heterogeneity concerning the intraoperative management of combined GA and EA.
Methods
This was an anonymous survey among Members of the Scientific working group for regional anesthesia within the German Society of Anaesthesiology and Intensive Care Medicine (DGAI) conducted from February 2020 to August 2020.
Results
The response rate was 38%. The majority of participants were experienced anesthetists with high expertise for the specific regimen of combined GA and EA. Most participants establish EA in the sitting position (94%), prefer early epidural initiation (prior to skin incision: 80%; intraoperative: 14%) and administer ropivacaine (89%) in rather low concentrations (0.2%: 45%; 0.375%: 30%; 0.75%: 15%) mostly with an opioid (84%) in a bolus-based mode (95%). The majority reduce systemic opioid doses intraoperatively if EA works sufficiently (minimal systemic opioids: 58%; analgesia exclusively via EA: 34%). About 85% manage intraoperative EA insufficiency with systemic opioids, 52% try to escalate EA, and only 25% use non-opioids, e.g. intravenous ketamine or lidocaine.
Conclusions
Although, consensus seems to be present for several aspects (patient's position during epidural puncture, main epidural substance, application mode), there is considerable heterogeneity regarding systemic opioids, rescue strategies for insufficient EA, and hemodynamic management, which might explain inconsistent results of previous trials and meta-analyses.
Häufig stellt die frühe Phase der innerklinischen Traumaversorgung bei Polytraumapatienten besonders die Schädel-Hirn-Verletzungen betreffend einen sehr dynamischen Zeitraum dar, weshalb zeitlich getriggerte Kontrolluntersuchungen weit verbreitet sind. Andererseits bedeutet der Transport zur Untersuchung für beatmete Patienten eine zusätzliche Gefahr. Untersucht wurde, wie häufig sich in einer frühen Kontroll-Computertomografie des Schädels neue oder verschlechterte Befunde finden und, wie diese neuen Informationen die Intensivtherapie beeinflussen. Methode: Retrospektive Analyse aus Befunddaten und Krankenakten. Eingeschlossen wurden 150 beatmete Patienten mit SHT oder Verdacht auf ein SHT, die eine frühe CCT-Kontrolle bekamen. Ergebnisse: Eine Verschlechterung der Befunde wurde bei 63 Patienten (42%) und neue Befunde bei 18 Patienten (12%) gefunden. Bei 47 Pateinten (31%) wurde die Intensivtherapie aufgrund der frühen Kontroll-CCT geändert. Schlussfolgerung: Bei beatmeten Polytraumapatienten treten in mehr als der Hälfte der Fälle Verschlechterungen oder neue intracranielle Befunde auf. Eine frühe CCT-Kontrolle kann diese erkennen und zu einer rechtzeitigen Therapieoptimierung führen.
Hintergrund: Bei der Einleitung einer Narkose eines nicht-nüchternen Patienten wird das Relaxanz in unmittelbarer Folge zum Induktionsmittel appliziert. Welches der Hypnotika Propofol, Etomidat oder Methohexital führt am schnellsten zur ausreichend tiefen Narkose?
Material/Methoden: Standardisierte Narkoseinduktion, je 20 ASA1/2 Patienten pro Medikament. Bestimmung Schlaftiefe mittels CSI: 1. vor Beginn , nach Midazolamgabe, 2. zwei Min. nach Fentanylgabe, 3. direkt nach Gabe des Hypnotikum (Dauer Injektion 1Min.), 4. nach 1 Min.
Propofol 3 mg/kg KG, Etomidat 0,3 mg/kg KG, Methohexital 1 mg/kg KG. Fentanyl 3 µg/kg KG, Midazolam 7,5mg
Ergebnisse: CSI direkt nach Injektion bei Methohexital mit 70,80 (+/-13,93) niedriger (p=0,024) als bei Propofol mit 81,05 (+/-10,82) und als bei Etomidat (p=0,026) mit 81,45 (+/-12,04). Abnahme des CSI während der Injektion bei Methohexital mit -9,6 (+/-12,14) größer (p=0,006) als bei Propofol mit –0,1 (+/-8,31). CSI <60 direkt nach Injektion bei Propofol 2x, Etomidat 3x, Methohexital 4x , nach einer Minute Propofol 17x, Etomidat 10x, Methohexital 16x.
Schlussfolgerung: Methohexital ermöglicht eine schnellere Narkoseinduktion als Etomidat und Propofol.
Background: Septic acute liver and intestinal failure is associated with a high mortality. We therefore investigated the influence of volume resuscitation with different crystalloid or colloid solutions on liver and intestine injury and microcirculation in septic rodents. Methods: Sepsis was induced by cecal ligation and puncture (CLP) in 77 male rats. Animals were treated with different crystalloids (NaCl 0.9% (NaCl), Ringer’s acetate (RA)) or colloids (Gelafundin 4% (Gel), 6% HES 130/0.4 (HES)). After 24 h animals were re-anesthetized and intestinal (n = 6/group) and liver microcirculation (n = 6/group) were obtained using intravital microscopy, as well as macrohemodynamic parameters were measured. Blood assays and organs were harvested to determine organ function and injury. Results: HES improved liver microcirculation, cardiac index and DO2-I, but significantly increased IL-1β, IL-6 and TNF-α levels and resulted in a mortality rate of 33%. Gel infused animals revealed significant reduction of liver and intestine microcirculation with severe side effects on coagulation (significantly increased PTT and INR, decreased haemoglobin and platelet count). Furthermore Gel showed severe hypoglycemia, acidosis and significantly increased ALT and IL-6 with a lethality of 29%. RA exhibited no derangements in liver microcirculation when compared to sham and HES. RA showed no intestinal microcirculation disturbance compared to sham, but significantly improved the number of intestinal capillaries with flow compared to HES. All RA treated animals survided and showed no severe side effects on coagulation, liver, macrohemodynamic or metabolic state. Conclusions: Gelatine 4% revealed devastated hepatic and intestinal microcirculation and severe side effects in CLP induced septic rats, whereas the balanced crystalloid solution showed stabilization of macro- and microhemodynamics with improved survival. HES improved liver microcirculation, but exhibited significantly increased pro-inflammatory cytokine levels. Crystalloid infusion revealed best results in mortality and microcirculation, when compared with colloid infusion.
Balanced hydroxyethylstarch (HES 130/0.4) impairs kidney function in-vivo without inflammation

Volume therapy is a standard procedure in daily perioperative care, and there is an ongoing discussion about the benefits of colloid resuscitation with hydroxyethylstarch (HES). In sepsis HES should be avoided due to a higher risk for acute kidney injury (AKI). Results of the usage of HES in patients without sepsis are controversial. Therefore we conducted an animal study to evaluate the impact of 6% HES 130/0.4 on kidney integrity with sepsis or under healthy conditions Sepsis was induced by standardized Colon Ascendens Stent Peritonitis (sCASP). sCASP-group as well as control group (C) remained untreated for 24 h. After 18 h sCASP+HES group (sCASP+VOL) and control+HES (C+VOL) received 50 ml/KG balanced 6% HES (VOL) 130/0.4 over 6h. After 24h kidney function was measured via Inulin- and PAH-Clearance in re-anesthetized rats, and serum urea, creatinine (crea), cystatin C and Neutrophil gelatinase-associated lipocalin (NGAL) as well as histopathology were analysed. In vitro human proximal tubule cells (PTC) were cultured +/- lipopolysaccharid (LPS) and with 0.1–4.0% VOL. Cell viability was measured with XTT-, cell toxicity with LDH-test. sCASP induced severe septic AKI demonstrated divergent results regarding renal function by clearance or creatinine measure focusing on VOL. Soleley HES (C+VOL) deteriorated renal function without sCASP. Histopathology revealed significantly derangements in all HES groups compared to control. In vitro LPS did not worsen the HES induced reduction of cell viability in PTC cells. For the first time, we demonstrated, that application of 50 ml/KG 6% HES 130/0.4 over 6 hours induced AKI without inflammation in vivo. Severity of sCASP induced septic AKI might be no longer susceptible to the way of volume expansion
Background
Up to 50% of septic patients develop acute kidney injury (AKI). The pathomechanism of septic AKI is poorly understood. Therefore, we established an innovative rodent model to characterize sepsis-induced AKI by standardized colon ascendens stent peritonitis (sCASP). The model has a standardized focus of infection, an intensive care set up with monitoring of haemodynamics and oxygenation resulting in predictable impairment of renal function, AKI parameters as well as histopathology scoring.
Methods
Anaesthetized rats underwent the sCASP procedure, whereas sham animals were sham operated and control animals were just monitored invasively. Haemodynamic variables and blood gases were continuously measured. After 24 h, animals were reanesthetized; cardiac output (CO), inulin and PAH clearances were measured and later on kidneys were harvested; and creatinine, urea, cystatin C and neutrophil gelatinase-associated lipocalin (NGAL) were analysed. Additional sCASP-treated animals were investigated after 3 and 9 days.
Results
All sCASP-treated animals survived, whilst ubiquitous peritonitis and significantly deteriorated clinical and macrohaemodynamic sepsis signs after 24 h (MAP, CO, heart rate) were obvious. Blood analyses showed increased lactate and IL-6 levels as well as leucopenia. Urine output, inulin and PAH clearance were significantly decreased in sCASP compared to sham and control. Additionally, significant increase in cystatin C and NGAL was detected. Standard parameters like serum creatinine and urea were elevated and sCASP-induced sepsis increased significantly in a time-dependent manner. The renal histopathological score of sCASP-treated animals deteriorated after 3 and 9 days.
Conclusions
The presented sCASP method is a standardized, reliable and reproducible method to induce septic AKI. The intensive care set up, continuous macrohaemodynamic and gas exchange monitoring, low mortality rate as well as the opportunity of detailed analyses of kidney function and impairments are advantages of this setup. Thus, our described method may serve as a new standard for experimental investigations of septic AKI.
Das Schädel-Hirn-Trauma (SHT) ist ein großes medizinisches Problem. Das Hirnödem mit konsekutiv erhöhten intrakraniellen Drücken ist eine häufige und schwerwiegende Komplikation des schweren SHT. Es ist der signifikanteste Prädiktor für ein schlechtes Outcome. Obwohl Glukokortikoide (GK) die Ausbildung eines Hirnödems bei neuroinflammatorischen Erkrankungen und manchen Hirntumoren reduzieren können, ist diese Substanzklasse im SHT ineffektiv oder sogar schädlich.
Nach controlled cortical impact (CCI) in Mäusen, einem etablierten SHT-Modell in-vivo, zeigte sich ein Zusammenbruch der Blut-Hirn-Schranke (BHS). Des Weiteren wurde der BHS-stabilisierende Effekt der GK nach CCI durch proteasomalen Abbau des Glukokortikoidrezeptors (GR) behindert. Eine Inhibierung des Proteasoms durch den Proteasomeninhibitor Bortezomib zusammen mit einer GK-Therapie mit Dexamethason reduzierte den Abbau des GR und sorgte für eine Restitution der BHS-Integrität.
Unglücklicherweise ließen sich diese Ergebnisse in-vitro nicht auf in Sauerstoff-/Glukosemangel-Modell in der humanen Hirnendothelzelllinie hCMEC/D3 übertragen. Daher konnte für die Kombinationstherapie aus dem Proteasomeninhibitor Bortezomib und Dexamethason kein positiver Effekt gezeigt werden.
Background:
Studies have reported on the incidence of sedation-related adverse events (AEs), but little is known about their impact on health care costs and resource use.
Methods: Health care providers and payers in five countries were recruited for an online survey by independent administrators to ensure that investigators and respondents were blinded to each other. Surveys were conducted in the local language and began with a "screener" to ensure that respondents had relevant expertise and experience. Responses were analyzed using Excel and R, with the Dixon's Q statistic used to identify and remove outliers. Global and country-specific average treatment patterns were calculated via bootstrapping; costs were mean values. The sum product of costs and intervention probability gave a cost per AE.
Results: Responses were received from 101 providers and 26 payers, the majority having. 5 years of experience. At a minimum, the respondents performed a total of 3,430 procedural sedations per month. All AEs detailed occurred in clinical practice in the last year and were reported to cause procedural delays and cancellations in some patients. Standard procedural sedation costs ranged from (sic)74 (Germany) to $2,300 (US). Respondents estimated that AEs would increase costs by between 16% (Italy) and 179% (US). Hypotension was reported as the most commonly observed AE with an associated global mean cost (interquartile range) of $43 ($27-$68). Other frequent AEs, including mild hypotension, bradycardia, tachycardia, mild oxygen desaturation, hypertension, and brief apnea, were estimated to increase health care spending on procedural sedation by $2.2 billion annually in the US.
Conclusion: All sedation-related AEs can increase health care costs and result in substantial delays or cancellations of subsequent procedures. The prevention of even minor AEs during procedural sedation may be crucial to ensuring its value as a health care service.
Rational drug design of Axl tyrosine kinase type I inhibitors as promising candidates against cancer

The high level of Axl tyrosine kinase expression in various cancer cell lines makes it an attractive target for the development of anti-cancer drugs. In this study, we carried out several sets of in silico screening for the ATP-competitive Axl kinase inhibitors based on different molecular docking protocols. The best drug-like candidates were identified, after parental structure modifications, by their highest affinity to the target protein. We found that our newly designed compound R5, a derivative of the R428 patented analog, is the most promising inhibitor of the Axl kinase according to the three molecular docking algorithms applied in the study. The molecular docking results are in agreement with the molecular dynamics simulations using the MM-PBSA/GBSA implicit solvation models, which confirm the high affinity of R5 toward the protein receptor. Additionally, the selectivity test against other kinases also reveals a high affinity of R5 toward ABL1 and Tyro3 kinases, emphasizing its promising potential for the treatment of malignant tumors.
