The benefits of cochlear implantation in children with severe hearing impairments are widely known; however, there is no consensus regarding which minimal outcome measurements (MOMs) should be used to determine outcomes in this population with pediatric cochlear implant (CI). Therefore, the authors aim to propose a MOM test battery for pediatric CI recipients that can facilitate international multi-center research and collaboration. A pediatric MOM test battery was developed and agreed-upon by members of the HEARRING group across 30 expert clinics in the field of hearing implantation. The MOM test battery was chosen based on a literature search that focused on outcome measurements applied in clinical trials involving children with a hearing implant. Members of the HEARRING group were then asked to evaluate each of the pediatric MOM tests used. The final pediatric MOM test battery was defined for different chronological age categories (six weeks–18 years) at different suggested test intervals. The test battery includes objective hearing measurements, aided and unaided audiometry, speech perception tests in quiet and in noise, subjective hearing assessments, assessment of language development, and mental and motor development. This study presents a consensus on a MOM test battery for pediatric CI recipients that was agreed upon by members of the HEARRING group. This test battery should allow for international multi-center research to be able to extend and share evidence that will guide future clinical practice and research efforts in pediatric populations with CI.
Purpose
While [18F]-fluorodeoxyglucose ([18F]FDG) is the standard for positron emission tomography/computed tomography (PET/CT) imaging of oral squamous cell carcinoma (OSCC), diagnostic specificity is hampered by uptake in inflammatory cells such as neutrophils or macrophages. Recently, molecular imaging probes targeting fibroblast activation protein α (FAP), which is overexpressed in a variety of cancer-associated fibroblasts, have become available and might constitute a feasible alternative to FDG PET/CT.
Methods
Ten consecutive, treatment-naïve patients (8 males, 2 females; mean age, 62 ± 9 years) with biopsy-proven OSCC underwent both whole-body [18F]FDG and [68Ga]FAPI-04 (FAP-directed) PET/CT for primary staging prior to tumor resection and cervical lymph node dissection. Detection of the primary tumor, as well as the presence and number of lymph node and distant metastases was analysed. Intensity of tracer accumulation was assessed by means of maximum (SUVmax) and peak (SUVpeak) standardized uptake values. Histological work-up including immunohistochemical staining for FAP served as standard of reference.
Results
[18F]FDG and FAP-directed PET/CT detected all primary tumors with a SUVmax of 25.5 ± 13.2 (FDG) and 20.5 ± 6.4 (FAP-directed) and a SUVpeak of 16.1 ± 10.3 ([18F]FDG) and 13.8 ± 3.9 (FAP-directed), respectively. Regarding cervical lymph node metastases, FAP-directed PET/CT demonstrated comparable sensitivity (81.3% vs. 87.5%; P = 0.32) and specificity (93.3% vs. 81.3%; P = 0.16) to [18F]FDG PET/CT. FAP expression on the cell surface of cancer-associated fibroblasts in both primary lesions as well as lymph nodes metastases was confirmed in all samples.
Conclusion
FAP-directed PET/CT in OSCC seems feasible. Future research to investigate its potential to improve patient staging is highly warranted.
Ag- but not ZnO-nanoparticles disturb the airway epithelial barrier at subtoxic concentrations

Inhalation is considered to be the most relevant source of human exposure to nanoparticles (NPs); however, only a few investigations have addressed the influence of exposing the respiratory mucosal barrier to subcytotoxic doses. In the nasal respiratory epithelium, cells of the mucosa represent one of the first contact points of the human organism with airborne NPs. Disruption of the epithelial barrier by harmful materials can lead to inflammation in addition to potential intrinsic toxicity of the particles. The aim of this study was to investigate whether subtoxic concentrations of zinc oxide (ZnO)- and silver (Ag)-NPs have an influence on upper airway barrier integrity. Nasal epithelial cells from 17 donors were cultured at the air–liquid interface and exposed to ZnO- and Ag-NPs. Barrier function, quantified by transepithelial electrical resistance (TEER), decreased after treatment with 10 µg/mL Ag-NPs, but FITC-dextran permeability remained stable and no change in mRNA levels of tight junction proteins and E-cadherin was detected by real-time quantitative PCR (RT-qPCR). The results indicate that subtoxic concentrations of Ag-NPs may already induce damage of the upper airway epithelial barrier in vitro. The lack of similar disruption by ZnO-NPs of similar size suggests a specific effect by Ag-NPs.
Neural stem cells (NSCs) have previously been described up to the adult stage in the rat cochlear nucleus (CN). A decreasing neurogenic potential was observed with critical changes around hearing onset. A better understanding of molecular factors affecting NSCs and neurogenesis is of interest as they represent potential targets to treat the cause of neurologically based hearing disorders. The role of genes affecting NSC development and neurogenesis in CN over time on hearing capacity has remained unclear. This study investigated the mRNA abundance of genes influencing NSCs and neurogenesis in rats’ CN over time. The CN of rats on postnatal days 6, 12, and 24 were examined. Real-time quantitative polymerase chain reaction arrays were used to compare mRNA levels of 84 genes relevant to NSCs and neurogenesis. Age- and hearing-specific patterns of changes in mRNA abundance of neurogenically relevant genes were detected in the rat CN. Additionally, crucial neurogenic factors with significant and relevant influence on neurogenesis were identified. The results of this work should contribute to a better understanding of the molecular mechanisms underlying the neurogenesis of the auditory pathway.
Das Vestibularisschwannom ist ein Tumor, dessen Entstehung noch nicht vollständig geklärt ist. Jeder kann von dieser seltenen Erkrankung betroffen sein. Darum ist es wichtig, die Ergebnisse der verschiedenen Therapieoptionen regelmäßig aufzuarbeiten und die Möglichkeiten einer Hörrehabilitation mit konventionellen oder implantierbaren Hörsystemen sowie Ansätze zur Schwindelrehabilitation oder einer möglichen Prähabilitation zu evaluieren. Mit der transtemporalen mikrochirurgischen Tumorexstirpation kann eine Heilung und bei einem großen Teil der Patienten auch ein Hörerhalt erzielt werden. Je jünger die Patienten sind, aber vor allem auch je besser sie vor der Operation hören, desto höher sind auch die Chancen auf einen Hörerhalt. Es zeigte sich jedoch auch, dass bei Patienten mit initial schlechterem Hören, welche dann operiert wurden, dieses zum Teil wieder verbessert werden konnte. Daher sind Ansätze, mehr Patienten eine transtemporale Tumorresektion anzubieten durchaus sinnvoll, auch um die Chancen auf eine Hörverbesserung zu ermöglichen. Des Weiteren sollte auch bei Patienten mit Schwindelbeschwerden eine Operation als Behandlungsoption erwogen werden. Die Prähabilitation mit Gentamicin-Injektionen in das Mittelohr scheint eine neue Möglichkeit zu sein, das zentrale Nervensystem schon auf den Ausfall des Vestibularorgans vorzubereiten. Durch die ototoxische Wirkung des Gentamicins und den potenziellen Hörverlust, wäre jedoch eine Kombination dieser Prähabilitation mit einem hörerhaltenden Eingriff über den transtemporalen Zugang risikobehaftet. Eine zentrale Kompensation der Schwindelbeschwerden sollte nach dem Eingriff mit einer gezielten Schwindelrehabilitation unterstützt werden. Während bei dieser vor allem physiotherapeutische Übungen angewandt werden, gibt es im Hinblick auf eine Hörrehabilitation schon verschiedene medizintechnische Optionen. Aktuell gibt es vor allem Studien zu den implantierbaren Hörhilfen und neue Daten zu den konservativen Möglichkeiten einer Hörrehabilitation sind eher die Ausnahme. Die Nutzung einer konservativen Hörhilfe sollte jedoch vor dem Entscheid zu einer operativen Lösung über implantierbare Systeme konsequent angewandt werden. Die in dieser Arbeit aufgezeigten positiven Momente bei der Versorgung von VS-Patienten prä- und posttherapeutisch mit herkömmlichen Hörgeräten sollten weiteren Eingang in die klinische Routine finden.
Salivary gland tumors (SGTs) are a relevant, highly diverse subgroup of head and neck tumors whose entity determination can be difficult. Confocal Raman imaging in combination with multivariate data analysis may possibly support their correct classification. For the analysis of the translational potential of Raman imaging in SGT determination, a multi-stage evaluation process is necessary. By measuring a sample set of Warthin tumor, pleomorphic adenoma and non-tumor salivary gland tissue, Raman data were obtained and a thorough Raman band analysis was performed. This evaluation revealed highly overlapping Raman patterns with only minor spectral differences. Consequently, a principal component analysis (PCA) was calculated and further combined with a discriminant analysis (DA) to enable the best possible distinction. The PCA-DA model was characterized by accuracy, sensitivity, selectivity and precision values above 90% and validated by predicting model-unknown Raman spectra, of which 93% were classified correctly. Thus, we state our PCA-DA to be suitable for parotid tumor and non-salivary salivary gland tissue discrimination and prediction. For evaluation of the translational potential, further validation steps are necessary.
Automated analysis of the inner ear anatomy in radiological data instead of time-consuming manual assessment is a worthwhile goal that could facilitate preoperative planning and clinical research. We propose a framework encompassing joint semantic segmentation of the inner ear and anatomical landmark detection of helicotrema, oval and round window. A fully automated pipeline with a single, dual-headed volumetric 3D U-Net was implemented, trained and evaluated using manually labeled in-house datasets from cadaveric specimen (N = 43) and clinical practice (N = 9). The model robustness was further evaluated on three independent open-source datasets (N = 23 + 7 + 17 scans) consisting of cadaveric specimen scans. For the in-house datasets, Dice scores of 0.97 and 0.94, intersection-over-union scores of 0.94 and 0.89 and average Hausdorf distances of 0.065 and 0.14 voxel units were achieved. The landmark localization task was performed automatically with an average localization error of 3.3 and 5.2 voxel units. A robust, albeit reduced performance could be
attained for the catalogue of three open-source datasets. Results of the ablation studies with 43 mono-parametric variations of the basal architecture and training protocol provided task-optimal parameters for both categories. Ablation studies against single-task variants of the basal architecture showed a clear performance beneft of coupling landmark localization with segmentation and a dataset-dependent performance impact on segmentation ability.
Seed amplification assays (SAA) are becoming commonly used in synucleinopathies to detect α-synuclein aggregates. Studies in Parkinson’s disease (PD) and isolated REM-sleep behavior disorder (iRBD) have shown a considerably lower sensitivity in the olfactory epithelium than in CSF or skin. To get an insight into α-synuclein (α-syn) distribution within the nervous system and reasons for low sensitivity, we compared SAA assessment of nasal brushings and skin biopsies in PD (n = 27) and iRBD patients (n = 18) and unaffected controls (n = 30). α-syn misfolding was overall found less commonly in the olfactory epithelium than in the skin, which could be partially explained by the nasal brushing matrix exerting an inhibitory effect on aggregation. Importantly, the α-syn distribution was not uniform: there was a higher deposition of misfolded α-syn across all sampled tissues in the iRBD cohort compared to PD (supporting the notion of RBD as a marker of a more malignant subtype of synucleinopathy) and in a subgroup of PD patients, misfolded α-syn was detectable only in the olfactory epithelium, suggestive of the recently proposed brain-first PD subtype. Assaying α-syn of diverse origins, such as olfactory (part of the central nervous system) and skin (peripheral nervous system), could increase diagnostic accuracy and allow better stratification of patients.
Purpose
While [\(^{18}\)F]-fluorodeoxyglucose ([\(^{18}\)F]FDG) is the standard for positron emission tomography/computed tomography (PET/CT) imaging of oral squamous cell carcinoma (OSCC), diagnostic specificity is hampered by uptake in inflammatory cells such as neutrophils or macrophages. Recently, molecular imaging probes targeting fibroblast activation protein α (FAP), which is overexpressed in a variety of cancer-associated fibroblasts, have become available and might constitute a feasible alternative to FDG PET/CT.
Methods
Ten consecutive, treatment-naïve patients (8 males, 2 females; mean age, 62 ± 9 years) with biopsy-proven OSCC underwent both whole-body [\(^{18}\)F]FDG and [\(^{68}\)Ga]FAPI-04 (FAP-directed) PET/CT for primary staging prior to tumor resection and cervical lymph node dissection. Detection of the primary tumor, as well as the presence and number of lymph node and distant metastases was analysed. Intensity of tracer accumulation was assessed by means of maximum (SUV\(_{max}\)) and peak (SUV\(_{peak}\) standardized uptake values. Histological work-up including immunohistochemical staining for FAP served as standard of reference.
Results
[\(^{18}\)F]FDG and FAP-directed PET/CT detected all primary tumors with a SUVmax of 25.5 ± 13.2 (FDG) and 20.5 ± 6.4 (FAP-directed) and a SUVpeak of 16.1 ± 10.3 ([\(^{18}\)F]FDG) and 13.8 ± 3.9 (FAP-directed), respectively. Regarding cervical lymph node metastases, FAP-directed PET/CT demonstrated comparable sensitivity (81.3% vs. 87.5%; P = 0.32) and specificity (93.3% vs. 81.3%; P = 0.16) to [\(^{18}\)F]FDG PET/CT. FAP expression on the cell surface of cancer-associated fibroblasts in both primary lesions as well as lymph nodes metastases was confirmed in all samples.
Conclusion
FAP-directed PET/CT in OSCC seems feasible. Future research to investigate its potential to improve patient staging is highly warranted.
Background: The benefit of hearing rehabilitation is often measured using audiological tests or subjective questionnaires/interviews. It is important to consider both aspects in order to evaluate the overall benefits. Currently, there is no standardized method for reporting combined audiological and patient reported subjective outcome measures in clinical practice. Therefore, this study focuses on showing the patient’s audiological, as well as subjective outcomes in one graph using data from an existing study. Method: The present paper illustrated a graph presenting data on four quadrants with audiological and subjective findings. These quadrants represented speech comprehension in quiet (unaided vs. aided) as WRS% at 65 dB SPL, speech recognition in noise (unaided vs. aided) as SRT dB SNR, sound field threshold (unaided vs. aided) as PTA\(_4\) in dB HL, wearing time and patient satisfaction questionnaire results. Results: As an example, the HEARRING graph in this paper represented audiological and subjective datasets on a single patient level or a cohort of patients for an active bone conduction hearing implant solution. The graph offered the option to follow the user’s performance in time. Conclusion: The HEARRING graph allowed representation of a combination of audiological measures with patient reported outcomes in one single graph, indicating the overall benefit of the intervention. In addition, the correlation and consistency between some results (e.g., aided threshold and aided WRS) can be better visualized. Those users who lacked performance benefits on one or more parameters and called for further insight could be visually identified.
Different effects of conditional Knock-Out of Stat3 on the sensory epithelium of the Organ of Corti

The mammalian cochlea detects sound in response to vibration at frequency-dependent positions along the cochlea duct. The sensory outer hair cells, which are surrounded by supporting cells, act as a signal amplifier by changing their cell length. This is called electromotility. To ensure correct electrical transmission during mechanical forces, a certain resistance of the sensory epithelium is a prerequisite for correct transduction of auditory information. This resistance is managed by microtubules and its posttranslational modification in the supporting cells of the sensory epithelium of the cochlea. Stat3 is a transcription factor, with its different phosphorylation sites, is involved in many cellular processes like differentiation, inflammation, cell survival and microtubule dynamics, depending on cell type and activated pathway. While Stat3 has a wide range of intracellular roles, the question arose, how and if Stat3 is involved in cells of the organ of Corti to ensure a correct hearing.
To test this, Cre/loxp system were used to perform conditional Knock-Out (cKO) of Stat3 in outer hair cells or supporting cells either before hearing onset or after hearing onset. Hearing performances included DPOAE and ABR measurements, while molecular were performed by sequencing. Additionally, morphological examination was used by immunohistochemistry and electron microscopy.
A cKO of Stat3 before and after hearing onset in outer hair cells leads to hearing impairments, whereas synapses, nerve fibers and mitochondria were not affected. Bulk sequencing analyzation of outer hair cells out of cKO mice before hearing onset resulted in a disturbance of cellular homeostasis and extracellular signals. A cKO of Stat3 in the outer hair cells after hearing onset resulted in inflammatory signaling pathway with increased cytokine production and upregulation of NF-kb pathway. In supporting cells, cKO of Stat3 only after hearing onset resulted in a hearing impairment. However, synapses, nerve soma and fibers were not affected of a cKO of Stat3 in supporting cells. Nevertheless, detyronisated modification of microtubules were altered, which can lead to an instability of supporting cells during hearing.
In conclusion, Stat3 likely interact in a cell-specific and function-specific manner in cells of the organ of Corti. While a cKO in outer hair cells resulted in increased cytokine production, supporting cells altered its stability due to decreased detyronisated modification of microtubules. Together the results indicated that Stat3 is an important protein for hearing performances. However, additional investigations of the molecular mechanism are needed to understand the role of Stat3 in the cells of the organ of Corti.
Background
Eye movement abnormalities are commonplace in neurological disorders. However, unaided eye movement assessments lack granularity. Although videooculography (VOG) improves diagnostic accuracy, resource intensiveness precludes its broad use. To bridge this care gap, we here validate a framework for smartphone video-based nystagmography capitalizing on recent computer vision advances.
Methods
A convolutional neural network was fine-tuned for pupil tracking using > 550 annotated frames: ConVNG. In a cross-sectional approach, slow-phase velocity of optokinetic nystagmus was calculated in 10 subjects using ConVNG and VOG. Equivalence of accuracy and precision was assessed using the “two one-sample t-test” (TOST) and Bayesian interval-null approaches. ConVNG was systematically compared to OpenFace and MediaPipe as computer vision (CV) benchmarks for gaze estimation.
Results
ConVNG tracking accuracy reached 9–15% of an average pupil diameter. In a fully independent clinical video dataset, ConVNG robustly detected pupil keypoints (median prediction confidence 0.85). SPV measurement accuracy was equivalent to VOG (TOST p < 0.017; Bayes factors (BF) > 24). ConVNG, but not MediaPipe, achieved equivalence to VOG in all SPV calculations. Median precision was 0.30°/s for ConVNG, 0.7°/s for MediaPipe and 0.12°/s for VOG. ConVNG precision was significantly higher than MediaPipe in vertical planes, but both algorithms’ precision was inferior to VOG.
Conclusions
ConVNG enables offline smartphone video nystagmography with an accuracy comparable to VOG and significantly higher precision than MediaPipe, a benchmark computer vision application for gaze estimation. This serves as a blueprint for highly accessible tools with potential to accelerate progress toward precise and personalized Medicine.
Die Maus mit progressiver motorischer Neuropathie (PMN) ist ein Modell für eine vererbte motorische Neuropathie mit progressiver Neurodegeneration. Die Degeneration der Axone geht mit homozygoten Mutationen des TBCE- Gens einher, das für das Tubulin-Chaperon-E- Protein kodiert. TBCE ist für die korrekte Dimerisierung von Alpha- und Beta-Tubulin verantwortlich. Auffallend ist, dass die PMN-Maus nach dem normalen Beginn des Hörens auch einen progressiven Hörverlust entwickelt, der durch die Degeneration des Hörnervs und den Verlust der äußeren Haarzellen (OHC) gekennzeichnet ist. Wirkung von peg-IGF-1 auf das auditorische System durch Behandlung ab dem 15. postnatalen Tag (p15). Die histologische Analyse ergab positive Auswirkungen auf die OHC-Synapsen der medialen olivocochleären (MOC) neuronalen Fasern und eine kurzfristige Abschwächung des OHC-Verlustes. Peg-IGF-1 war in der Lage, die Desorganisation der OHC-Synapsen bedingt wiederherzustellen und die Bereitstellung von cholinerger Acetyltransferase in den Präsynapsen aufrechtzuerhalten. Zur Beurteilung der auditorischen Funktion wurden frequenzspezifische Hirnstammreaktionen und otoakustische Emissionen mit Verzerrungsprodukten bei Tieren mit p21 und p28 aufgezeichnet. Trotz der positiven Auswirkungen auf die MOC-Fasern und die OHC konnte jedoch keine Wiederherstellung des Hörvermögens erreicht werden. Die vorliegende Arbeit zeigt, dass die synaptische Pathologie der efferenten MOC-Fasern in PMN-Mäusen eine besondere Form der "efferenten auditorischen Neuropathie" darstellt. Peg-IGF-1 zeigte eine otoprotektive Wirkung, indem es die Degeneration von OHCs und efferenten Synapsen verhinderte. Es sind jedoch verstärkte Anstrengungen zur Optimierung der Behandlung erforderlich, um nachweisbare Verbesserungen der Hörleistung zu erzielen.
The medial geniculate body (MGB) is a nucleus of the diencephalon representing a relevant segment of the auditory pathway and is part of the metathalamus. It receives afferent information via the inferior brachium of the inferior colliculus and transmits efferent fibers via acoustic radiations to the auditory cortex. Neural stem cells (NSCs) have been detected in certain areas along the auditory pathway. They are of great importance as the induction of an adult stem cell niche might open a regenerative approach to a causal treatment of hearing disorders. Up to now, the existence of NSCs in the MGB has not been determined. Therefore, this study investigated whether the MGB has a neural stem cell potential. For this purpose, cells were extracted from the MGB of PND 8 Sprague-Dawley rats and cultured in a free-floating cell culture assay, which showed mitotic activity and positive staining for stem cell and progenitor markers. In differentiation assays, the markers β-III-tubulin, GFAP, and MBP demonstrated the capacity of single cells to differentiate into neuronal and glial cells. In conclusion, cells from the MGB exhibited the cardinal features of NSCs: self-renewal, the formation of progenitor cells, and differentiation into all neuronal lineage cells. These findings may contribute to a better understanding of the development of the auditory pathway.
Due to the wide variety of benign and malignant salivary gland tumors, classification and malignant behavior determination based on histomorphological criteria can be difficult and sometimes impossible. Spectroscopical procedures can acquire molecular biological information without destroying the tissue within the measurement processes. Since several tissue preparation procedures exist, our study investigated the impact of these preparations on the chemical composition of healthy and tumorous salivary gland tissue by Fourier-transform infrared (FTIR) microspectroscopy. Sequential tissue cross-sections were prepared from native, formalin-fixed and formalin-fixed paraffin-embedded (FFPE) tissue and analyzed. The FFPE cross-sections were dewaxed and remeasured. By using principal component analysis (PCA) combined with a discriminant analysis (DA), robust models for the distinction of sample preparations were built individually for each parotid tissue type. As a result, the PCA-DA model evaluation showed a high similarity between native and formalin-fixed tissues based on their chemical composition. Thus, formalin-fixed tissues are highly representative of the native samples and facilitate a transfer from scientific laboratory analysis into the clinical routine due to their robust nature. Furthermore, the dewaxing of the cross-sections entails the loss of molecular information. Our study successfully demonstrated how FTIR microspectroscopy can be used as a powerful tool within existing clinical workflows.
Purpose
Surgery is a standard therapy for tympanojugular paragangliomas (TJP). Maintaining the quality of life (QoL) requires functional preservation. The flexible CO\(_2\) laser allows contact-free tumor removal. This retrospective study compares the postoperative functional outcomes of TJP surgery with and without the flexible CO\(_2\) laser.
Methods
Between 2005 and 2019, 51 patients with TJP were surgically treated at a tertiary hospital. Until 2012, 17 patients received conventional surgery. Thereafter, the flexible laser was used in 34 patients. Tumor extend, pre- and postoperative cranial nerve function, and complications were compared between the groups.
Results
The cohort consisted of 33 class A and B tumors and 18 class C and D tumors. Preoperative embolization was performed in 17 cases. Class C/D TJP were usually removed via an infratemporal fossa type A approach. Gross total tumor removal was achieved in 14/18 class C/D tumors. 3/51 patients suffered from long-term partial or complete facial palsy. No differences in post-therapeutic cranial nerve function or complications were noted between the conventional and laser group. One recurrence was observed after complete tumor resection.
Conclusion
The flexible CO\(_2\) laser was shown to be a safe and effective alternative to conventional bipolar cauterization, which is appreciated by the surgeon in these highly vascularized tumors. Both techniques allowed a high tumor control rate and good long-term results also from a functional point of view.
Surgical management of lateral neck abscesses in children: a retrospective analysis of 100 cases

Cervical abscesses are relatively common infections in pediatric patients. There is an ongoing debate about the necessity and time point of surgical drainage. The identification of a focus of infection might play an important role in facilitating a therapeutic decision. In a retrospective study, 100 pediatric patients aged 1–18 years who underwent incision and drainage of a lateral cervical abscess at our institution were analyzed. Patients were divided into two groups based on whether a focus of infection could be identified or not. Data collection included patient characteristics, microbiological results, antibiotic regimen, and clinical course. A focus of infection was found in 29% (29/100) of the patients, most frequently in the tonsils. A causative microorganism was found in 75% (75/100) of all patients, with Staphylococcus aureus and Streptococcus pyogenes being the most common pathogens. All patients received an empiric antibiotic therapy in addition to surgery. Antibiotic medication was changed in 31% in both groups (9/29 with a focus of infection and 22/71 without a focus of infection) during therapy. Children without an identified focus of infection generally were younger and had more comorbidities reducing immune response while also showing differences in the pathogens involved. There were no complications associated to surgery or antibiotic therapy in any of the patients involved.
Conclusion: Children with an identified focus of infection show several differences compared to those with isolated lateral abscesses, especially regarding the microorganisms involved. But the focus of infection seems not to have an impact on patient’s outcome.

Towards a consensus on an ICF-based classification system for horizontal sound-source localization

The study aimed to develop a consensus classification system for the reporting of sound localization testing results, especially in the field of cochlear implantation. Against the background of an overview of the wide variations present in localization testing procedures and reporting metrics, a novel classification system was proposed to report localization errors according to the widely accepted International Classification of Functioning, Disability and Health (ICF) framework. The obtained HEARRING_LOC_ICF scale includes the ICF graded scale: 0 (no impairment), 1 (mild impairment), 2 (moderate impairment), 3 (severe impairment), and 4 (complete impairment). Improvement of comparability of localization results across institutes, localization testing setups, and listeners was demonstrated by applying the classification system retrospectively to data obtained from cohorts of normal-hearing and cochlear implant listeners at our institutes. The application of our classification system will help to facilitate multi-center studies, as well as allowing better meta-analyses of data, resulting in improved evidence-based practice in the field.
Trotz langjähriger klinischer Erfahrung, vielen Untersuchungen und Studien, ist die korrekte Diagnosestellung eines Morbus Menière immer noch schwierig. Diagnostische Mittel sind vor allem eine ausführliche Anamnese, die ECOG, Tonaudiogramm und der Ausschluss anderer Innenohrerkrankungen. Betroffene erleiden häufig Schwindelanfälle, Tinnitus, Ohrdruck und verlieren an Hörvermögen. Der progressive Verlauf dieser Erkrankung lässt sich meist nur verlangsamen. Häufig startet die Therapie funktionserhaltend-medikamentös mit Betahistin und Antiemetika, im weiteren Verlauf sind meist operative Eingriffe wie die Saccotomie nötig. Dabei wird der Saccus endolymphaticus aufgesucht, geschlitzt und mittels Silikondreieck offengehalten, um damit eine Entlastung des endolymphatischen Systems zu schaffen. Zeigt sich nur wenig Besserung kann destruierend fortgefahren werden. Hierbei sind vor allem die Gentamycin-Therapie und die Neurektomie oder Labyrinthektomie Mittel der Wahl.
Diese Untersuchung hatte das Ziel eine weitere diagnostische Methode zu finden, um die MM-Diagnose zu sichern und eine Prognose in Bezug auf die Therapie mittels Saccotomie zu liefern. Im Fokus stand hierbei der vestibuläre Aquädukt, welcher bereits in vielen Arbeiten auffällig in Form und Lage bei MM-Patienten gewesen war. Vor allem der Bezug des Aquäduktes zu den Bogengängen und dessen Länge wurden in der vorliegenden Studie genauer betrachtet. Dies geschah durch Messungen anhand von CT-Bildern. Der Winkel zwischen den Bogengängen und dem VA und die Länge des VA wurde gemessen und ausgewertet. Dies wurde zur Bestätigung der Methode, sowohl an menschlichen Felsenbeinpräparaten mit qualitativ unterschiedlichen CT-Aufnahmen als auch an Patienten durchgeführt. Dabei konnte man keine signifikanten Unterschiede innerhalb der verschiedenen CT-Aufnahmen der Felsenbeinpräparate feststellen.
Mit Bestätigung der Reliabilität dieser Methode wurden dann Patienten untersucht. Dabei wurden zwei Gruppen gebildet, Nicht-MM- und MM-Patienten. Die Nicht-MM bestanden aus Patienten mit einem Akustikusneurinom der Gegenseite oder einem Cochlea-Implantat, welche keinen MM aufwiesen. Die MM-Patienten wurden alle mit einer Saccotomie behandelt und hatten damit einen gesicherten MM. Die Auswertung der Messergebnisse stellte dar, dass bei MM-Betroffenen die Länge mit durchschnittlich 7,9 mm im Vergleich mit Nicht-Betroffenen-VA (9,5 mm), signifikant reduziert war. Auch die Ergebnisse der Winkelmessung zeigte bei allen Bogengängen signifikante Unterschiede.
Um die Ergebnisse der Winkelmessung besser zu kategorisieren, wurde der VA-Score erstellt. Dieser vereinigt die Ergebnisse der drei Bogengänge zu einem Wert von 1 bis 8. Auch hier erwiesen sich die MM-Patienten als auffällig, da sich diese im VA-Score 2 mit 44% häuften. Im Gegensatz dazu waren die Ergebnisse der nicht Erkrankten breiter verteilt.
Zur weiteren Auswertung zählten außerdem die anamnestischen Daten und der Verlauf der Saccotomie. Dadurch konnte der Erfolg der Behandlung bestimmt und diese mit den Untersuchungen des VA des jeweiligen Patienten korreliert werden. Dabei stellte sich heraus, dass vor allem bei VA mit hohen Winkeln und sehr kurzen VA die Erfolgsquote geringer war.
Damit könnte also ein Zusammenhang zwischen Form und Lage des VA und Therapie bestehen. Um dies sicher zu bestätigen, braucht es noch mehr Patienten und eine klinisch zugelassene Software, die diese Messungen durchführen kann. Hierbei ist vor allem eine gute Bildgebung des Innenohrs wichtig, um den VA optimal messen zu können. Um die Messungen schnell und zuverlässig durchführen zu können, wäre eine Software, welche die Messungen automatisch durchführen kann, ein möglicher Ansatz. Die ausführliche Voruntersuchung wird jedoch weiterhin notwendig sein, um MM sicher zu diagnostizieren.
Sprachaudiometrische Untersuchungen sind Voraussetzung für die korrekte Verordnung und Anpassung von Hörgeräten und Cochlea-Implantaten sowie der arbeitsmedizinischen Begutachtung von Minderungen der Erwerbsfähigkeit. Um das Kommunikationsvermögen im lärmbelasteten Alltag zu prüfen, eignen sich Sprachverständlichkeitsmessungen im Störschall. In dieser Arbeit sollten mit dem HSM-Satztest Normwerte zum Sprachverstehen im Störlärm an jungen Erwachsenen für Messungen an Hörgeschädigten ermittelt werden. Junge Erwachsene sind i.d.R. maximal konzentrations- und leistungsfähig sowie normal hörend. Ferner wurde überprüft, inwiefern sich das individuelle Antwortverhalten der Probanden und die Position der Satzgruppe auf das Sprachverstehen auswirken. Dreißig Probanden beiderlei Geschlechts im Alter von 20 bis 29 Jahren wurden zwischen beiden Ohren wechselnd digital aufgezeichnete, sinnvolle Sätze in Störschallpegeln von 60 und 80 dB dargeboten. Die getrennte Betrachtung der Ergebnisse bei 60 und 80 dB Störschall zeigte für den Bereich der Sprachverständlichkeitsschwelle ein signifikant besseres Sprachverstehen bei 80 dB Störschall. Hier war ein geringerer Sprachschallpegel zum fünfzigprozentigen Sprachverständnis erforderlich. Außerdem war bei gleichem Signal- Rausch-Abstand das prozentuale Verständnis höher. Die Befunde mit der binaural alternierenden Darbietung des HSM-Tests bestätigen entsprechende Vermutungen früherer Untersuchungen von Hörgeräteträgern und älteren Normalhörenden. Daher wurde nicht ein Artefakt des hier verwendeten Versuchsaufbaus gemessen. Forsches Antwortverhalten zeigte eine leichte, jedoch nicht signifikante Tendenz zu besser bewertetem Sprachverständnis. Unterschiede im Sprachverständnis in Abhängigkeit von der Position der Satzgruppe konnten nicht festegestellt werden. Die Definition der Norm mit diesem Testansatz, der Alltagsbedingungen im Labor simuliert, ist eine wesentliche Voraussetzung für präzise Diagnostik von Hörstörungen und ihre optimierte Behandlung.
Für die Cochlea-Implantat-Versorgung ist die Kenntnis der individuellen Anatomie der Hörschnecke im perioperativen Kontext essenziell, um ein suffizientes audiologisches Resultat sicherzustellen. Ein akkurates Verfahren hierfür stellt die 3D multiplanare Rekonstruktion (3D-curved MPR) in Schnittbildgebung dar. Notwendige Voraussetzung ist jedoch eine hinreichende Bildqualität. In dieser Arbeit wurde das Augenmerk auf die sekundäre Rekonstruktion von Primärdatensätzen der Flachdetektor-Volumen-Computertomographie (fpVCTSECO) gerichtet. Diese bietet nämlich die Möglichkeit, die Bildqualität zu steigern, ohne jedoch eine als kritisch einzuschätzende Dosissteigerung in Kauf nehmen zu müssen.
Es konnte gezeigt werden, dass es für die Messung der Länge von 2 Schneckenwindungen (2TL), der gesamten cochleären Länge (CDL) und dem Winkelmaß (AL) mittels 3D-curved MPR in der fpVCT keinen signifikanten Unterschied gegenüber der Mehrschicht-CT gibt. In beiden Modalitäten wurden alle drei Parameter gegenüber der Referenzbildgebung micro-CT deutlich unterschätzt. Durch die fpVCTSECO war es möglich, die Genauigkeit der Messungen zu steigern und den Werten der Referenz anzunähern. Lediglich für AL muss eine geringfügige systematische Unterschätzung beachtet werden.
Postoperativ zeigte sich mit einliegendem Elektrodenträger für 2TL eine ebenso präzise Messung wie präoperativ ohne. Jedoch wurde die CDL um circa 0,5 - 0,7 mm unterschätzt. Ursächlich hierfür dürften vor allem Metallartefakte gewesen sein. Auch wenn die 3D-curved MPR in Kombination mit der fpVCTSECO postoperativ zur Visualisierung der räumlichen Beziehung von Elektrodenträger, Modiolus und ossärer lateraler Wand sehr gut geeignet war, so muss sich der Einfluss dieser Diskrepanz für die audiologische Anpassung in Zukunft erst noch zeigen.
Eine retrospektive Analyse laryngealer Vorläuferläsionen sowie deren Verlauf und Progressionsrisiko

Hinter dem makroskopischen Bild einer Leukoplakie der Stimmlippen können sich verschiedene histopathologische Diagnosen, wie Hyper- oder Parakeratosen, unterschiedliche Dysplasieschweregrade oder ein invasives Karzinom, verbergen. Die Diagnose wird durch Exzision und histopathologische Beurteilung gestellt, gefolgt von einer Einteilung je nach Klassifikationssystem. Die existierenden Klassifikationssysteme sind in ihrer Aussagekraft bezüglich des Progressionsrisikos der verschiedenen Vorläuferläsionen und der daraus resultierenden Behandlungsempfehlung eingeschränkt. Die neue Einteilung der WHO aus dem Jahr 2017 unterscheidet „low-grade“ Dysplasien (ehemals Epithelhyperplasien und leichte Dysplasie) von „high-grade“ Dysplasien (ehemals mäßige- und schwergradige Dysplasien einschließlich des Carcinoma in situ).
In der vorliegenden Arbeit wurden insgesamt 392 Patienten mit laryngealen Vorläuferläsionen aus der Klinik und Poliklinik für Hals-, Nasen- und Ohrenheilkunde des Universitätsklinikums Würzburg untersucht. Es waren insbesondere Männer im Durchschnittsalter von 59,9 Jahren betroffen. Zudem wird ein Raucheranteil von 85,1 % beschrieben.
Im Verlauf entwickelten 57 Patienten (14,5%) ein invasives Karzinom. Mit steigendem Dysplasieschweregrad konnte eine zunehmende Entartungstendenz beobachtet werden. Patienten mit der initialen Diagnose einer Hyper- oder Parakeratose ohne Dysplasie (5,6%) bzw. einer leichtgradigen Dysplasie (8,9%) wiesen ein signifikant geringeres Entartungsrisiko auf als Patienten mit höhergradigen dysplastischen Veränderungen (p<0,001). Mäßiggradige (41%) und schwergradige Dysplasien (43,5%) bzw. Carcinomata in situ (54,5%) wiesen ein vergleichbar hohes Progressionsrisiko auf. Mäßige Dysplasien wurden in bisherigen Arbeiten bezüglich ihres Entartungsrisikos eher unterschätzt und oftmals mit den leichtgradigen Dysplasien in einer Gruppe zusammengefasst. Die aktuell erhobenen Daten weisen jedoch auf ein höher als ursprünglich angenommenes Entartungsrisiko hin, sodass aufgrund des hohen Progressionsrisikos die Aufnahme in die Kategorie der „high-grade“ Dysplasien gerechtfertigt scheint. Es lässt sich zudem beobachten, dass der Zeitraum in dem sich aus einer schwergradigen Dysplasie (45 Wochen) bzw. einem Carcinoma in situ (66,2 Wochen) ein Larynxkarzinom entwickelt, kürzer ist als der der mäßigen Dysplasien (117,1 Woche).
Weitere Studien sind erforderlich, um die neu gewonnen Erkenntnisse zu validieren, das neue Klassifikationssystem der WHO 2017 in die klinische Praxis zu integrieren und ein besseres Verständnis der zugrunde liegenden Pathomechanismen zu entwickeln.
Tumore von Kopf und Hals gehen weiterhin mit einer schlechten Prognose einher. Im Rahmen einer operativen Therapie tritt Wundsekret (WS) aus, welches der Wundheilung dient. Dieses kann in Kontakt mit Tumorzellen bzw. Resttumor in der Wunde kommen.
Im Rahmen der vorliegenden Arbeit wurde die Frage nach dem Einfluss von Wundsekret auf Zellvermehrung, Chemoresistenzentwicklung, den Zellzyklus und die Induktion einer Epithelial-mesenchymalen Transition (EMT) in Tumorzellen von Kopf und Hals gestellt.
Hierfür wurde das WS von Tag1 und das WS von Tag 2 im Dotblot auf seine Zytokinzusammensetzung analysiert. Zwei Tumorzelllinien von Kopf und Hals, FaDu und HlaC78, wurden mit WSTag1 und WSTag2 behandelt und untersucht, welche Effekte das WS auf die Zellen hat.
Verwendet wurden ein Proliferationsassay, eine Zellzyklusuntersuchung und Apoptosetestung mittels FACS, eine PCR, ein Spheroidmodell und die Lichtmikroskopie.
Im WS wurden erhöhte Konzentrationen verschiedener Zytokine, insbesondere von IL-6, nachgewiesen. Gezeigt werden konnte eine gesteigerte Proliferationsrate der Tumorzellen unter WS-Behandlung, jedoch keine veränderte Verteilung der Zellzyklusphasen. In HlaC78-Zellen konnte eine vermehrte Vitalität nach Cisplatinbehandlung nachgewiesen werden. In beiden Tumorzelllinien fand sich eine vermehrte Exprimierung von Snail 1, Snail 2 und Vimentin. E-Cadherin wurde vermindert exprimiert. Twist und N-Cadherin wiesen keine Veränderungen auf. Es zeigte sich eine vermehrte Migration der Tumorzellen in die Umgebung. Die Zellen wiesen nach Behandlung mit WS vermehrt mesenchymale Zeichen auf. Es konnte kein Unterschied der Auswirkungen einer Behandlung mit WSTag1 im Vergleich zu einer Behandlung mit WSTag2 festgestellt werden.
Insgesamt scheint WS in Tumorzellen von Kopf und Hals einen EMT-artigen Prozess in Gang zu setzen, also eine partial EMT (pEMT).
Als mögliche Auslöser dieser Veränderungen kommen die im WS nachgewiesenen Zytokine und v. a. IL-6 in Frage.
This study aimed to discover expert opinion on the surgical techniques and materials most likely to achieve maximum postoperative residual hearing preservation in cochlear implant (CI) surgery and to determine how these opinions have changed since 2010. A previously published questionnaire used in a study published in 2010 was adapted and expanded. The questionnaire was distributed to an international group of experienced CI surgeons. Present results were compared, via descriptive statistics, to those from the 2010 survey. Eighteen surgeons completed the questionnaire. Respondents clearly favored the following: round window insertion, slow array insertion, and the peri- and postoperative use of systematic antibiotics. Insertion depth was regarded as important, and electrode arrays less likely to induce trauma were preferred. The usefulness of dedicated soft-surgery training was also recognized. A lack of agreement was found on whether the middle ear cavity should be flushed with a non-aminoglycoside antibiotic solution or whether a sheath or insertion tube should be used to avoid contaminating the array with blood or bone dust. In conclusion, this paper demonstrates how beliefs about CI soft surgery have changed since 2010 and shows areas of current consensus and disagreement.
Im Jahre 2011 wurden erstmals neuronale Stammzellen (NSCs) im Nucleus Cochlearis (N.C.) der Ratte beschrieben (Rak et al. 2011). Um diese Zellen besser zu charakterisieren, war das Ziel der vorliegenden Arbeit, die NSCs des N.C. im Hinblick auf ihre Maturation und Interaktion in neuronalen Netzwerken sowie auf die Möglichkeiten nichtinvasiver Beeinflussung dieser Zellen zu untersuchen und diese mit primären Neuronen des N.C. zu vergleichen. Für die Untersuchungen waren intrazelluläre Calcium-Ionen (Ca2+) von besonderem Interesse, da diese über spannungsgesteuerte Ca2+-Kanäle (VGCCs) und deren Spontanoszillationen indirekt die Aktivität und Differenzierung der Neurone widerspiegeln könnten.
Für die Analyse wurden N.C.s von P6 Ratten mikroskopisch präpariert und nach Dissoziation in Einzelzellen die NSCs für 8 Wochen in Stammzellmedium kultiviert oder direkt als primäre Neurone im Stammzellmedium ausplattiert.
Zur Vorbereitung der Untersuchungen fand eine Kultivierung der jeweiligen Zellen für 4 Tage in Differenzierungsmedium statt. Anschließend wurden sie für Calcium-Imaging-Messungen mit dem Ca2+-sensitiven Fluorophor Oregon Green BAPTA-1 beladen. Zum einen wurde eine Analyse der Grundaktivitäten innerhalb der Zellareale und im neuronalen Netzwerk im Verlauf der Maturation durchgeführt. Zum anderen fand am Tag 4 der Zelldifferenzierung (DIF d4) eine Untersuchung der qualitativen und quantitativen Verteilung von VGCCs über die Zugabe der Ca2+-Kanalinhibitoren Nifedipin, ω-Conotoxin MVIIC, Kurtoxin und SNX-482 statt. In jedem Fall wurden die Zellen anschließend mit PFA fixiert und immunzytologisch untersucht. Zudem wurde eine Markierung der VGCCs mit dem Antikörper anti-Ca2+-Channel-(1 Subunit)-Pan vorgenommen.
Innerhalb der Ergebnisse konnte eine Abhängigkeit der neuronalen Reifung von der Zellaktivität in Form von Ca2+-Strömen nachgewiesen werden. Hierfür zeigte sich ursächlich eine Variation im qualitativen und quantitativen Vorkommen von VGCCs und in ihrer Spontanaktivität innerhalb der Zellareale im Verlauf der neuronalen Maturation. NSCs zeigten ein ähnliches Verhalten wie primär kultivierte Neurone - sowohl bezüglich ihres Aktivitätsmusters während der Differenzierung als auch bezüglich ihrer Möglichkeit der Inhibierung, was auf eine ähnliche Expression von VGCCs hinweisen könnte. Die höchste Aktivität zeigte sich in beiden Fällen bei DIF d4. Die neurogene Nische, welche in der Literatur sowohl bei Ratten (Rak et al. 2011) als auch bei Mäusen (Volkenstein et al. 2013) im N.C. nachweisbar war, könnte somit zur Analysierung pathologischer Prozesse sowie auch zu deren Behandlung in Betracht gezogen werden.
Zusammenfassend konnte in der vorliegenden Dissertation eine Charakterisierung des N.C. der Hörbahn in elektrophysiologischer und biochemischer Hinsicht erreicht werden. Die Ansätze dieser Arbeit könnten in Zukunft zu Therapieoptionen der Hörrehabilitation auf dieser Ebene beitragen.
Die Nutzung mobiler Informations- und Kommunikationstechnologie im Kontext medizinscher und gesundheitsnaher Dienstleistungen, z. B. in Form von Apps, gewinnt, leider jedoch häufig unter Missachtung notwendiger Qualitätskriterien, immer mehr an Bedeutung. So erscheint es wichtig, dass neben einer die Anwendungssoftware kontrollierenden Instanz möglichst hoch qualifizierte Akteure des Gesundheitswesens bei der Erstellung mitwirken. Schon aus Haftungsgründen muss der beratende Arzt eine hohe Sorgfalt bei der Auswahl und Empfehlung einer App walten lassen, gerade auch in Anbetracht der Tatsache, dass nur wenige Apps als Medizinprodukt zertifiziert sind. Auf dem Markt gibt es eine große Anzahl an medizinischen Apps, wobei nur ein geringer Anteil dieser auf den Fachbereich HNO-Heilkunde entfällt. Prozentual sind die Teilbereiche Audiologie, Schlafmedizin und Allergologie am häufigsten vertreten. Obwohl sich der Fachbereich der HNO-Heilkunde zunehmend mit dieser Thematik wissenschaftlich auseinandersetzt, fehlt, wie generell bei vielen medizinischen Apps, eine wissenschaftliche Evidenz der Inhalte und Ergebnisse. Es gibt jedoch für Anwender weitere Möglichkeiten, medizinische Apps nach definierten Qualitätskriterien in verschiedenen Kategorien, wie z. B. Funktionalität, Wissenschaftlichkeit, aber auch Datenschutz, zu beurteilen. Keine der mittels eines solchen Bewertungstools evaluierten Apps erfüllte alle geforderten Kriterien, dem Anwender steht jedoch hiermit ein Instrument zur besseren Einschätzung der Anwendungssoftware zur Verfügung. Im Rahmen des Entwicklungsprozesses einer medizinischen App für den HNO-Bereich war es jedoch möglich, die Qualitätskriterien zu berücksichtigen. Zusammenfassend soll die vorliegende Arbeit einen Überblick über Thema „Apps in der HNO-Heilkunde“ ermöglichen mit dem Ziel, dieses moderne Hilfsmittel nutzbringend einsetzen zu können.
Background
Fabry Disease (FD) is an X-linked hereditary lysosomal storage disorder which leads to a multisystemic intralysosomal accumulation of globotriaosylceramid (Gb3). Besides prominent renal and cardiac organ involvement, patients commonly complain about vestibulocochlear symptoms like high-frequency hearing loss, tinnitus and vertigo. However, comprehensive data especially on vertigo remain scarce. The aim of this study was to examine the prevalence and characteristics of vertigo and hearing loss in patients with FD, depending on renal and cardiac parameters and get hints about the site and the pattern of the lesions.
Methods
Single-center study with 57 FD patients. Every patient underwent an oto-rhino-laryngological examination as well as videonystagmography and vestibular evoked myogenic potentials (VEMPs) and audiological measurements using pure tone audiometry and auditory brainstem response audiometry (ABR). Renal function was measured by eGFR, cardiac impairment was graduated by NYHA class.
Results
More than one out of three patients (35.1%) complained about hearing loss, 54.4% about vertigo and 28.1% about both symptom. In 74% a sensorineural hearing loss of at least 25 dB was found, ABR could exclude any retrocochlear lesion. Caloric testing showed abnormal values in 71.9%, VEMPs were pathological in 68%. A correlation between the side or the shape of hearing loss and pathological vestibular testing could not be revealed.
Conclusions
Hearing loss and vertigo show a high prevalence in FD. While hearing loss seems due to a cochlear lesion, peripheral vestibular as well as central nervous pathologies cause vertigo. Thus, both the site of lesion and the pathophysiological patterns seem to differ.
Improved radiological examinations with newly developed 3D models may increase understanding of Meniere's disease (MD). The morphology and course of the vestibular aqueduct (VA) in the temporal bone might be related to the severity of MD. The presented study explored, if the VA of MD and non-MD patients can be grouped relative to its angle to the semicircular canals (SCC) and length using a 3D model. Scans of temporal bone specimens (TBS) were performed using micro-CT and micro flat panel volume computed tomography (mfpVCT). Furthermore, scans were carried out in patients and TBS by computed tomography (CT). The angle between the VA and the three SCC, as well as the length of the VA were measured. From these data, a 3D model was constructed to develop the vestibular aqueduct score (VAS). Using different imaging modalities it was demonstrated that angle measurements of the VA are reliable and can be effectively used for detailed diagnostic investigation. To test the clinical relevance, the VAS was applied on MD and on non-MD patients. Length and angle values from MD patients differed from non-MD patients. In MD patients, significantly higher numbers of VAs could be assigned to a distinct group of the VAS. In addition, it was tested, whether the outcome of a treatment option for MD can be correlated to the VAS.
The progressive motor neuropathy (PMN) mouse is a model of an inherited motor neuropathy disease with progressive neurodegeneration. Axon degeneration associates with homozygous mutations of the TBCE gene encoding the tubulin chaperone E protein. TBCE is responsible for the correct dimerization of alpha and beta-tubulin. Strikingly, the PMN mouse also develops a progressive hearing loss after normal hearing onset, characterized by degeneration of the auditory nerve and outer hair cell (OHC) loss. However, the development of this neuronal and cochlear pathology is not fully understood yet. Previous studies with pegylated insulin-like growth factor 1 (peg-IGF-1) treatment in this mouse model have been shown to expand lifespan, weight, muscle strength, and motor coordination. Accordingly, peg-IGF-1 was evaluated for an otoprotective effect. We investigated the effect of peg-IGF-1 on the auditory system by treatment starting at postnatal day 15 (p15). Histological analysis revealed positive effects on OHC synapses of medial olivocochlear (MOC) neuronal fibers and a short-term attenuation of OHC loss. Peg-IGF-1 was able to conditionally restore the disorganization of OHC synapses and maintain the provision of cholinergic acetyltransferase in presynapses. To assess auditory function, frequency-specific auditory brainstem responses and distortion product otoacoustic emissions were recorded in animals on p21 and p28. However, despite the positive effect on MOC fibers and OHC, no restoration of hearing could be achieved. The present work demonstrates that the synaptic pathology of efferent MOC fibers in PMN mice represents a particular form of “efferent auditory neuropathy.” Peg-IGF-1 showed an otoprotective effect by preventing the degeneration of OHCs and efferent synapses. However, enhanced efforts are needed to optimize the treatment to obtain detectable improvements in hearing performances.
This proof of concept describes the use of evoked electromyographic (EMG) activation of the facial nerve for intraoperative monitoring of the electrode insertion during cochlear implantation (CI). Intraoperative EMG measurements from the facial nerve were conducted in nine patients undergoing CI implantation. Electric current pulses were emitted from contacts on the CI array during and immediately after electrode insertion. For control, the results of EMG measurements were compared to postoperative flat panel volume computed tomography scans with secondary reconstruction (fpVCT\(_{SECO}\)). During insertion, the EMG response evoked by the electrical stimulation from the CI was growing with the stimulating contact approaching the facial nerve and declined with increasing distance. After full insertion, contacts on the apical half of the CI array stimulated higher EMG responses compared with those on the basal half. Comparison with postoperative imaging demonstrated that electrode contacts stimulating high EMG responses had the shortest distances to the facial nerve. It could be demonstrated that electrically evoked EMG activation of the facial nerve can be used to monitor the progress during CI electrode insertion and to control the intracochlear electrode position after full insertion.
Adipose-derived stromal cells (ASCs) are a promising cell source for tissue engineering and regenerative medicine approaches for cartilage replacement. For chondrogenic differentiation, human (h)ASCs were seeded on three-dimensional polyurethane (PU) fibrin composites and induced with a chondrogenic differentiation medium containing TGF-ß3, BMP-6, and IGF-1 in various combinations. In addition, in vitro predifferentiated cell-seeded constructs were implanted into auricular cartilage defects of New Zealand White Rabbits for 4 and 12 weeks. Histological, immunohistochemical, and RT-PCR analyses were performed on the constructs maintained in vitro to determine extracellular matrix (ECM) deposition and expression of specific cartilage markers. Chondrogenic differentiated constructs showed a uniform distribution of cells and ECM proteins. RT-PCR showed increased gene expression of collagen II, collagen X, and aggrecan and nearly stable expression of SOX-9 and collagen I. Rabbit (r)ASC-seeded PU-fibrin composites implanted in ear cartilage defects of New Zealand White Rabbits showed deposition of ECM with structures resembling cartilage lacunae by Alcian blue staining. However, extracellular calcium deposition became detectable over the course of 12 weeks. RT-PCR showed evidence of endochondral ossification during the time course with the expression of specific marker genes (collagen X and RUNX-2). In conclusion, hASCs show chondrogenic differentiation capacity in vitro with the expression of specific marker genes and deposition of cartilage-specific ECM proteins. After implantation of predifferentiated rASC-seeded PU-fibrin scaffolds into a cartilage defect, the constructs undergo the route of endochondral ossification.

Zinc oxide nanoparticles (ZnO-NP) are commonly used for a variety of applications in everyday life. In addition, due to its versatility, nanotechnology supports promising approaches in the medical sector. NP can act as drug-carriers in the context of targeted chemo- or immunotherapy, and might also exhibit autonomous immune-modulatory characteristics. Knowledge of potential immunosuppressive or stimulating effects of NP is indispensable for the safety of consumers as well as patients. In this study, primary human peripheral blood lymphocytes of 9 donors were treated with different sub-cytotoxic concentrations of ZnO-NP for the duration of 1, 2, or 3 days. Flow cytometry was performed to investigate changes in the activation profile and the proportion of T cell subpopulations. ZnO-NP applied in this study did not induce any significant alterations in the examined markers, indicating their lack of impairment in terms of immune modulation. However, physicochemical characteristics exert a major influence on NP-associated bioactivity. To allow a precise simulation of the complex molecular processes of immune modulation, a physiological model including the different components of an immune response is needed.
Neuronale Stammzellen wurden kürzlich im unteren Colliculus inferior (CI) identifiziert. Diese Zellen sind von besonderem Interesse, da es keine therapeutischen Optionen für geschädigte neuronale Strukturen gibt. Ziel dieses Forschungsprojekts ist es, das neurogene Potenzial im CI der Ratte von den ersten postnatalen Tagen bis zum Erwachsenenalter zu untersuchen. Der CI von Ratten vom 6. bis zum 48. postnatalen Tag wurde mit Neurosphären-Assays und histologischen Schnitten untersucht. In frei schwimmenden CI-Zellkulturen bildeten sich Neurosphären bei Tieren vom frühen postnatalen Alter bis zum Erwachsenenalter. Die Menge der gebildeten Neurosphären nahm im höheren Alter ab und stieg mit der Anzahl der Zelllinienpassagen. Die Zellen in den Neurosphären und die histologischen Schnitte zeigten eine positive Färbung mit neuronalen Stammzell-Markern (Doublecortin, Sox-2, Musashi-1, Nestin und Atoh1). Dissoziierte Einzelzellen aus den Neurosphären differenzierten und wurden positiv für die neuralen Abstammungsmarker β-III-Tubulin, GFAP und MBP angefärbt. Darüber hinaus wurden neuronalen Stammzell-Marker (Doublecortin, Sox-2, CDK5R1 und Ascl-1) mittels qRT-PCR untersucht. Zusammenfassend lässt sich sagen, dass ein neurogenes Potenzial im CI der Ratte von den frühen postnatalen Tagen bis zum Erwachsenenalter nachgewiesen und bewertet wurde. Die Identifizierung von neuronalen Stammzellen im CI der Ratte und ihre altersspezifischen Merkmale tragen zu einem besseren Verständnis der Entwicklung und der Plastizität der Hörbahn bei und könnten für eine therapeutische Nutzung aktiviert werden.
Adipose tissue-derived stromal cells (ASCs) represent a capable source for cell-based therapeutic approaches. For monitoring a cell-based application in vivo, magnetic resonance imaging (MRI) of cells labeled with iron oxide particles is a common method. It is the aim of the present study to analyze potential DNA damage, cytotoxicity and impairment of functional properties of human (h)ASCs after labeling with citrate-coated very small superparamagnetic iron oxide particles (VSOPs). Cytotoxic as well as genotoxic effects of the labeling procedure were measured in labeled and unlabeled hASCs using the MTT assay, comet assay and chromosomal aberration test. Trilineage differentiation was performed to evaluate an impairment of the differentiation potential due to the particles. Proliferation as well as migration capability were analyzed after the labeling procedure. Furthermore, the labeling of the hASCs was confirmed by Prussian blue staining, transmission electron microscopy (TEM) and high-resolution MRI. Below the concentration of 0.6 mM, which was used for the procedure, no evidence of genotoxic effects was found. At 0.6 mM, 1 mM as well as 1.5 mM, an increase in the number of chromosomal aberrations was determined. Cytotoxic effects were not observed at any concentration. Proliferation, migration capability and differentiation potential were also not affected by the procedure. Labeling with VSOPs is a useful labeling method for hASCs that does not affect their proliferation, migration and differentiation potential. Despite the absence of cytotoxicity, however, indications of genotoxic effects have been demonstrated.
Vorliegende Arbeit beschäftigte sich mit der Verbesserung von Defiziten der elektrischen Stimulation durch Cochlea Implantate (CI) mit alternativen Pulsformen. Dabei wurde mit elektrophysiologischen und psychophysikalischen Methoden untersucht, wie sich die Pulsformen auf die Effektivität der Stimulation auswirken. Es wurden präzisions-triphasische Pulse (pTP) mit anodischer und kathodischer zweiter Phase anhand der Daten von elf Probanden untersucht.
Im Rahmen der objektiven elektrophysiologischen Messung wurde mit den unterschiedlichen Formen des pTP an drei unterschiedlichen Kontaktpositionen auf den CI-Elektrodenträgern stimuliert, und die Stärke der jeweils evozierten neuronalen Antwort aufgezeichnet. Der subjektive psychophysikalische Test diente dazu, die pulsformspezifischen Hörschwellen zu bestimmen und wurde an zwei unterschiedlichen Kontakten auf den CI-Elektrodenträgern durchgeführt.
Dabei erzielten pTP, welche eine symmetrisch-triphasische Pulsform aufwiesen, geringere neuronale Antwortstärken und höhere Hörschwellen als die pTP, die einer biphasischen Pulsform glichen. Diejenigen pTP, die biphasischen Pulsen mit anodischer erster Phase glichen, erzielten dabei die höchsten neuronalen Antwortstärken und die niedrigsten Hörschwellen.
Hintergrund
Die E‑Zigarette erfreut sich in den letzten Jahren zunehmender Beliebtheit. Die Frage nach der Toxizität ist jedoch noch nicht eindeutig geklärt, und es herrscht global Unsicherheit im Umgang mit der E‑Zigarette.
Ziel
Ziel der vorliegenden Arbeit war es, Propylenglykol, ein Hauptbestandteil der Liquide, in Bezug auf mögliche akute Entzündungsreaktionen, zyto- und genotoxische Auswirkungen auf humane Nasenschleimhautzellen zu untersuchen.
Material und Methoden
Die Nasenschleimhautzellen wurden von zehn Probanden im Air-Liquid-Interface kultiviert und anschließend mit unterschiedlichen Konzentrationen des Propylenglykols bedampft. Die Analyse erfolgte mittels Trypanblau-Test, Comet-Assay, Mikrokerntest und IL-6- und IL-8-Sandwich-ELISA.
Ergebnis
Der Trypanblau-Test zeigte keine Reduktion der Vitalität. Im Sandwich-ELISA konnte kein Anstieg der IL-6- und IL-8-Konzentrationen nachgewiesen werden. Im Comet-Assay zeigte das Olive Tail Moment eine Schädigung im Vergleich zur Negativkontrolle in allen untersuchten Konzentrationen. Zudem zeigte sich eine dosisabhängige Schädigung. Im Mikrokerntest konnte ein Unterschied zwischen dem Reinstoff und der Negativkontrolle gefunden werden.
Schlussfolgerung
Es zeigten sich möglicherweise reparable DNS-Schädigungen im Comet-Assay. Im Mikrokerntest konnten diese nur in der Reinstoffkonzentration bestätigt werden. Es sollte ein restriktiver Umgang mit der E‑Zigarette erfolgen, bis insbesondere Langzeitstudien vorliegen. Zudem ist eine eindeutige Deklaration der Inhaltsstoffe der Liquide durch die Hersteller zu fordern, um weitergehende Schädigungspotenziale untersuchen zu können.
Zinc oxide nanoparticles (ZnO-NPs) are widely utilized, for example in manufacturing paints and in the cosmetic industry. In addition, there is raising interest in the application of NPs in stem cell research. However, cytotoxic, genotoxic and pro-inflammatory effects were shown for NPs. The aim of this study was to evaluate the impact of ZnO-NPs on cytokine secretion and differentiation properties of human adipose tissue-derived stromal cells (ASCs). Human ASCs were exposed to the subtoxic concentration of 0.2 mu g/mL ZnO-NPs for 24 h. After four weeks of cultivation, adipogenic and osteogenic differentiation procedures were performed. The multi-differentiation potential was confirmed histologically and using polymerase chain reaction (PCR). In addition, the gene expression of IL-6, IL-8, vascular endothelial growth factor (VEGF) and caspase 3 was analyzed. Over the course of four weeks after ZnO-NPs exposure, no significant differences were detected in the gene expression of IL-6, IL-8, VEGF and caspase 3 compared to non-exposed cells. The differentiation was also not affected by the ZnO-NPs. These findings underline the fact, that functionality of ASCs is likely to be unaffected by ZnO-NPs, despite a long-term disposition of NPs in the cells, supposing that the starting concentration was safely in the non-toxic range. This might provide important information for single-use nanomedical applications of ZnO-NPs.
Entwicklung optimierter Ankopplungsbedingungen von aktiven Mittelohrimplantaten am runden Fenster

Seit Mitte der 90er-Jahre gewinnen aktive Mittelohrimplantate wie die Vibrant sound bridge (VSB) zunehmend an Bedeutung. Sie können zur Rehabilitation von Schallleitungs- und Schallempfindungsstörungen, sowie kombinierter Hörstörungen indiziert sein. Die Vibrant sound bridge stellt eine Alternative für Patienten dar, bei denen aus verschiedenen Gründen eine Unverträglichkeit gegenüber herkömmlichen Hörgeräten besteht. Bei der Implantation des Floating mass transducers (FMT) am runden Fenster zeigte sich in der Klinik neben einer hohen Revisionsrate von bis zu 15,6 % eine hohe Varianz der audiologischen Ergebnisse, was auf eine unkontrollierte und mangelnde Ankopplung des Floating mass transducers an das runde Fenster zurückzuführen ist. Erste Ergebnisse der kontrollierten Ankopplung am runden Fenster mit einem Kamin zeigten vielversprechende Ergebnisse. Die suffiziente Fixation des Kamins an der Rundfensternische stellt eine Herausforderung dar. Es bedarf eines Haftvermittlers, der eine suffiziente Verbindung zwischen dem Titankamin und dem darunter befindlichen Knochen am runden Fenster herstellt. Ziel der Dissertation war die Findung einer geeigneten Haftkomponente zur Fixation des Kamins am runden Fenster. Es wurden der Calciumphosphatzement HydroSet® (Fa. Stryker) und der Glasionomerzement Ketac™ Cem (Fa. ESPE 3M™ Deutschland) untersucht. In Anlehnung an DIN 13990-2 wurden nach Einwirkung physikalischen Stresses in Form von Temperatur und Feuchtigkeit die Scherhaftfestigkeit der Titanzylinder an humanen Felsenbeinknochen gemessen. Es zeigte sich eine schwach signifikant bessere Haftung für das Calciumphosphatzement HydroSet®, bei insgesamt geringer Scherhaftfestigkeit beider Zemente im Vergleich mit anderen zahnheilkundlichen Adhäsiven der Literatur.
Zinkoxid-Nanopartikel (ZnO-NP) finden in vielen Produkten des täglichen Verbrauchs Verwendung. Daten über die toxikologischen Eigenschaften von ZnO-NP werden kontrovers diskutiert. Die menschliche Haut ist in Bezug auf die ZnO-NP Exposition das wichtigste Kontakt-Organ. Intakte Haut stellt eine suffiziente Barriere gegenüber NP dar. Bei defekter Haut ist ein Kontakt zu den proliferierenden Stammzellen möglich, sodass diese als wichtiges toxikologische Ziel für NP darstellen. Das Ziel dieser Dissertation war die Bewertung der genotoxischen und zytotoxischen Effekte an humanen mesenchymalen Stammzellen (hMSC) durch niedrig dosierte ZnO-NP nach 24 stündiger Exposition, repetitiven Expositionen und im Langzeitversuch bis zu 6 Wochen. Zytotoxische Wirkungen von ZnO-NP wurden mit 3-(4,5-Dimethylthiazol-2-yl)-2,5-diphenyltetrazoliumbromid-Test (MTT) gemessen. Darüber hinaus wurde die Genotoxizität durch den Comet-Assay bewertet. Zur Langzeitbeobachtung bis zu 6 Wochen wurde die Transmissionselektronenmikroskopie (TEM) verwendet. Zytotoxizität nach 24-stündiger ZnO-NP-Exposition war ab einer Konzentration von 50 µg/ml nachweisbar. Genotoxizität konnten bereits bei Konzentrationen von 1 und 10 µg/ml ZnO-NP beschrieben werden. Wiederholte Exposition verstärkte die Zyto-, aber nicht die Genotoxizität. Eine intrazelluläre NP-Akkumulation mit Penetration der Zellorganelle wurde bei einer Exposition bis zu 6 Wochen beobachtet. Die Ergebnisse deuten auf zytotoxische und genotoxisches Effekte von ZnO-NP hin. Bereits geringe Dosen von ZnO-NP können bei wiederholter Exposition toxische Wirkungen hervorrufen sowie eine langfristige Zellakkumulation. Diese Daten sollten bei der Verwendung von ZnO-NP an geschädigter Haut berücksichtigt werden.
Magnetic nanoparticles (NPs), such as very small iron oxide NPs (VSOPs) can be used for targeted drug delivery, cancer treatment or tissue engineering. Another important field of application is the labelling of mesenchymal stem cells to allow in vivo tracking and visualization of transplanted cells using magnetic resonance imaging (MRI). For these NPs, however, various toxic effects, as well as functional impairment of the exposed cells, are described. The present study evaluates the influence of VSOPs on the multilineage differentiation ability and cytokine secretion of human adipose tissue derived stromal cells (hASCs) after long-term exposure. Human ASCs were labelled with VSOPs, and the efficacy of the labelling was documented over 4 weeks in vitro cultivation of the labelled cells. Unlabelled hASCs served as negative controls. Four weeks after labelling, adipogenic and osteogenic differentiation was histologically evaluated and quantified by polymerase chain reaction (PCR). Changes in gene expression of IL-6, IL-8, VEGF and caspase 3 were determined over 4 weeks. Four weeks after the labelling procedure, labelled and unlabelled hASCs did not differ in the gene expression of IL-6, IL-8, VEGF and caspase 3. Furthermore, the labelling procedure had no influence on the multidifferentiation ability of hASC. The percentage of labelled cells decreased during in vitro expansion over 4 weeks. Labelling with VSOPs and long-term intracellular disposition probably have no influence on the physiological functions of hASCs. This could be important for the future in vivo use of iron oxide NPs.
Purpose
Flexible endoscopic evaluation of swallowing (FEES\(^{®}\)) is a standard diagnostic tool in dysphagia. The combination of FEES® and narrow band light (narrow band imaging; NBI) provides a more precise and detailed investigation method. So far, this technique could only be performed with the NBI illumination. The new version of the "professional image enhancement technique" (PIET) provides another image enhancing system. This study investigates the eligibility of PIET in the FEES\(^{®}\) procedure.
Methods
Both techniques, NBI and PIET, were compared using a target system. Furthermore, the image enhancement during FEES\(^{®}\) was performed and recorded with the two systems during daily routine.
Results
Performing an image enhancement during FEES\(^{®}\) is possible with both systems PIET and NBI. On the target system, the contrast of the PIET showed a brighter and a more detailed picture. In dysphagia patients, no difference between PIET and NBI was detected.
Conclusion
PIET proved to be non-inferior to NBI during image enhancement FEES\(^{®}\). So far, image enhancement FEES\(^{®}\) was exclusively connected to NBI. With the PIET system, an alternative endoscopy technology is available for certain indications.
Deep neck infections with and without mediastinal involvement: treatment and outcome in 218 patients

Purpose
Infections of the deep neck, although becoming scarcer due to the widespread use of antibiotics, still represent a dangerous and possibly deadly disease, especially when descending into the mediastinum. Due to the different specialities involved in the treatment and the heterogenous presentation of the disease, therapeutic standard is still controversial. This study analyzes treatment and outcome in these patients based on a large retrospective review and proposes a therapeutic algorithm.
Methods
The cases of 218 adult patients treated with deep neck abscesses over a 10-year period at a tertiary university hospital were analyzed retrospectively. Clinical, radiological, microbiological and laboratory findings were compared between patients with and without mediastinal involvement.
Results
Forty-five patients (20.64%) presented with abscess formation descending into the mediastinum. Those patients had significantly (all items p < 0.0001) higher rates of surgical interventions (4.27 vs. 1.11) and tracheotomies (82% vs. 3.4%), higher markers of inflammation (CRP 26.09 vs. 10.41 mg/dl), required more CT-scans (3.58 vs. 0.85), longer hospitalization (39.78 vs 9.79 days) and more frequently needed a change in antibiotic therapy (44.44% vs. 6.40%). Multi-resistant pathogens were found in 6.67% vs. 1.16%. Overall mortality rate was low with 1.83%.
Conclusion
Despite of the high percentage of mediastinal involvement in the present patient collective, the proposed therapeutic algorithm resulted in a low mortality rate. Frequent CT-scans, regular planned surgical revisions with local drainage and lavage, as well as an early tracheotomy seem to be most beneficial regarding the outcome.
Purpose
For further improvements in cochlear implantation, the measurement of the cochlear duct length (CDL) and the determination of the electrode contact position (ECP) are increasingly in the focus of clinical research. Usually, these items were investigated by multislice computed tomography (MSCT). The determination of ECP was only possible by research programs so far. Flat-panel volume computed tomography (fpVCT) and its secondary reconstructions (fpVCT\(_{SECO}\)) allow for high spatial resolution for the visualization of the temporal bone structures. Using a newly developed surgical planning software that enables the evaluation of CDL and the determination of postoperative ECP, this study aimed to investigate the combination of fpVCT and otological planning software to improve the implementation of an anatomically based cochlear implantation.
Methods
Cochlear measurements were performed utilizing surgical planning software in imaging data (MSCT, fpVCT and fpVCT\(_{SECO}\)) of patients with and without implanted electrodes.
Results
Measurement of the CDL by the use of an otological planning software was highly reliable using fpVCT\(_{SECO}\) with a lower variance between the respective measurements compared to MSCT. The determination of the inter-electrode-distance (IED) between the ECP was improved in fpVCT\(_{SECO}\) compared to MSCT.
Conclusion
The combination of fpVCT\(_{SECO}\) and otological planning software permits a simplified and more reliable analysis of the cochlea in the pre- and postoperative setting. The combination of both systems will enable further progress in the development of an anatomically based cochlear implantation.
Purpose
Audiology is an essential service for some patient groups and some interventions. This article sets forth experience-based recommendations for how audiological centers can continue to safely and effectively function during COVID-19.
Methods
The recommendations are the result of panel discussion and are based on the clinical experience of the panelists/authors.
Results
The recommendations cover which patient groups and which interventions should be treated when and whether this can be performed in the clinic or remotely; how to maintain the safety of workplace via optimizing patient flow within the clinic and the sanitation of rooms and equipment; and overcoming communication challenges that COVID-19 intensifies.
Conclusion
For essential audiological services to continue under COVID-19, safety measures must be implemented and maintained, and treatment and communication strategies must be adapted to offset communication difficulties due to personal protective equipment (PPE) and social distancing and to bolster patient confidence. In short, it is vital that staff feel safe, that patients either feel the clinic is safe enough to visit or that remote treatment may be an option, and that clinics and patients have a broad agreement on the urgency of any needed service. We hope that these recommendations help clinics effectively accomplish these goals.
Pilot study on the value of Raman spectroscopy in the entity assignment of salivary gland tumors

Background
The entity assignment of salivary gland tumors (SGT) based on histomorphology can be challenging. Raman spectroscopy has been applied to analyze differences in the molecular composition of tissues. The aim of this study was to evaluate the suitability of RS for entity assignment in SGT.
Methods
Raman data were collected in deparaffinized sections of pleomorphic adenomas (PA) and adenoid cystic carcinomas (ACC). Multivariate data and chemometric analysis were completed using the Unscrambler software.
Results
The Raman spectra detected in ACC samples were mostly assigned to nucleic acids, lipids, and amides. In a principal component-based linear discriminant analysis (LDA) 18 of 20 tumor samples were classified correctly.
Conclusion
In this proof of concept study, we show that a reliable SGT diagnosis based on LDA algorithm appears possible, despite variations in the entity-specific mean spectra. However, a standardized workflow for tissue sample preparation, measurement setup, and chemometric algorithms is essential to get reliable results.
Objective
Growing interest in measuring the cochlear duct length (CDL) has emerged, since it can influence the selection of cochlear implant electrodes. Currently the measurements are performed with ionized radiation imaging. Only a few studies have explored CDL measurements in magnetic resonance imaging (MRI). Therefore, the presented study aims to fill this gap by estimating CDL in MRI and comparing it with multislice computed tomography (CT).
Study Design
Retrospective data analyses of 42 cochleae.
Setting
Tertiary care medical center.
Methods
Diameter (A value) and width (B value) of the cochlea were measured in HOROS software. The CDL and the 2-turn length were determined by the elliptic circular approximation (ECA). In addition, the CDL, the 2-turn length, and the angular length were determined via HOROS software by the multiplanar reconstruction (MPR) method.
Results
CDL values were significantly shorter in MRI by MPR (d = 1.38 mm, P < .001) but not by ECA. Similar 2-turn length measurements were significantly lower in MRI by MPR (d = 1.67 mm) and ECA (d = 1.19 mm, both P < .001). In contrast, angular length was significantly higher in MRI (d = 26.79°, P < .001). When the values were set in relation to the frequencies of the cochlea, no clinically relevant differences were estimated (58 Hz at 28-mm CDL).
Conclusion
In the presented study, CDL was investigated in CT and MRI by using different approaches. Since no clinically relevant differences were found, diagnostics with radiation may be omitted prior to cochlear implantation; thus, a concept of radiation-free cochlear implantation could be established.
This study aimed to evaluate the feasibility and accuracy of electromagnetic navigation at the lateral skull base in combination with flat panel volume computed tomography (fpVCT) datasets. A mastoidectomy and a posterior tympanotomy were performed on 10 samples of fresh frozen temporal bones. For registration, four self-drilling titanium screws were applied as fiducial markers. Multi-slice computed tomography (MSCT; 600 µm), conventional flat panel volume computed tomography (fpVCT; 466 µm), micro-fpVCT (197 µm) and secondary reconstructed fpVCT (100 µM) scans were performed and data were loaded into the navigation system. The resulting fiducial registration error (FRE) was analysed, and control of the navigation accuracy was performed. The registration process was very quick and reliable with the screws as fiducials. Compared to using the MSCT data, the micro-fpVCT data led to significantly lower FRE values, whereas conventional fpVCT and secondary reconstructed fpVCT data had no advantage in terms of accuracy. For all imaging modalities, there was no relevant visual deviation when targeting defined anatomical points with a navigation probe. fpVCT data are very well suited for electromagnetic navigation at the lateral skull base. The use of titanium screws as fiducial markers turned out to be ideal for comparing different imaging methods. A further evaluation of this approach by a clinical trial is required.
Objective
To evaluate changing trends in patient collectives, age-related patterns of manifestation, and diagnostic pathways of patients with extrapulmonary head and neck tuberculosis (TB), and to provide strategies to fasten diagnosis in these patients.

Case control study.
Methods
A 10-year retrospective analysis of 35 patients diagnosed with extrapulmonary TB in the head and neck at a tertiary university institution from 2009 to 2019, with special focus on the influence of the patient's age on consideration of TB and clinical patterns.
Results
The vast majority of patients younger than 40 years had their origin in countries with high TB burden (P = .0003), and TB was considered very early as a differential diagnosis (P = .0068), while most patients older than 40 years were domestic citizens initially suspected for a malignancy, who more often had an underlying immunosuppressive condition (0.0472). Most frequent manifestations in both groups were the lymph nodes, larynx, and oropharynx. Surprisingly, no differences in the rates of open TB or history of TB infection in the family anamnesis were found.
Conclusion
The two groups of patients found most often are younger patients migrating from regions with high TB burden and elderly domestic patients suffering from immunosuppressive conditions, with the latter often being misdiagnosed as malignancies. TB remains an important but difficult differential diagnosis, due to the initially unspecific symptoms and the great variety in the presentation of manifestations in the head and neck.
Locoregional recurrence is a major reason for therapy failure after surgical resection of head and neck squamous cell carcinoma (HNSCC). The physiological process of postoperative wound healing could potentially support the proliferation of remaining tumor cells. The aim of this study was to evaluate the influence of wound fluid (WF) on the cell cycle distribution and a potential induction of epithelial-mesenchymal transition (EMT). To verify this hypothesis, we incubated FaDu and HLaC78 cells with postoperative WF from patients after neck dissection. Cell viability in dependence of WF concentration and cisplatin was measured by flow cytometry. Cell cycle analysis was performed by flow cytometry and EMT-marker expression by rtPCR. WF showed high concentrations of interleukin (IL)-6, IL-8, IL-10, CCL2, MCP-1, EGF, angiogenin, and leptin. The cultivation of tumor cells with WF resulted in a significant increase in cell proliferation without affecting the cell cycle. In addition, there was a significant enhancement of the mesenchymal markers Snail 2 and vimentin, while the expression of the epithelial marker E-cadherin was significantly decreased. After cisplatin treatment, tumor cells incubated with WF showed a significantly higher resistance compared with the control group. The effect of cisplatin-resistance was dependent on the WF concentration. In summary, proinflammatory cytokines are predominantly found in WF. Furthermore, the results suggest that EMT can be induced by WF, which could be a possible mechanism for cisplatin resistance.
To study the interaction of human pathogens with their host target structures, human tissue models based on primary cells are considered suitable. Complex tissue models of the human airways have been used as infection models for various viral and bacterial pathogens. The Gram-negative bacterium Bordetella pertussis is of relevant clinical interest since whooping cough has developed into a resurgent infectious disease. In the present study, we created three-dimensional tissue models of the human ciliated nasal and tracheo-bronchial mucosa. We compared the innate immune response of these models towards the B. pertussis virulence factor adenylate cyclase toxin (CyaA) and its enzymatically inactive but fully pore-forming toxoid CyaA-AC\(^-\). Applying molecular biological, histological, and microbiological assays, we found that 1 µg/ml CyaA elevated the intracellular cAMP level but did not disturb the epithelial barrier integrity of nasal and tracheo-bronchial airway mucosa tissue models. Interestingly, CyaA significantly increased interleukin 6, interleukin 8, and human beta defensin 2 secretion in nasal tissue models, whereas tracheo-bronchial tissue models were not significantly affected compared to the controls. Subsequently, we investigated the interaction of B. pertussis with both differentiated primary nasal and tracheo-bronchial tissue models and demonstrated bacterial adherence and invasion without observing host cell type-specific significant differences. Even though the nasal and the tracheo-bronchial mucosa appear similar from a histological perspective, they are differentially susceptible to B. pertussis CyaA in vitro. Our finding that nasal tissue models showed an increased innate immune response towards the B. pertussis virulence factor CyaA compared to tracheo-bronchial tissue models may reflect the key role of the nasal airway mucosa as the first line of defense against airborne pathogens.
Causal therapies for the auditory-pathway and inner-ear diseases are still not yet available for clinical application. Regenerative medicine approaches are discussed and examined as possible therapy options. Neural stem cells could play a role in the regeneration of the auditory pathway. In recent years, neural stem and progenitor cells have been identified in the cochlear nucleus, the second nucleus of the auditory pathway. The current investigation aimed to analyze cell maturation concerning cellular calcium activity. Cochlear nuclei from PND9 CD rats were microscopically dissected and propagated as neurospheres in free-floating cultures in stem-cell medium (Neurobasal, B27, GlutaMAX, EGF, bFGF). After 30 days, the dissociation and plating of these cells took place under withdrawal of the growth factors and the addition of retinoic acid, which induces neural cell differentiation. Calcium imaging analysis with BAPTA-1/Oregon Green was carried out at different times during the differentiation phase. In addition, the influence of different voltage-dependent calcium channels was analyzed through the targeted application of inhibitors of the L-, N-, R- and T-type calcium channels. For this purpose, comparative examinations were performed on CN NSCs, and primary CN neurons. As the cells differentiated, a significant increase in spontaneous neuronal calcium activity was demonstrated. In the differentiation stage, specific frequencies of the spontaneous calcium oscillations were measured in different regions of the individual cells. Initially, the highest frequency of spontaneous calcium oscillations was ascertainable in the maturing somata. Over time, these were overtaken by calcium oscillations in the axons and dendrites. Additionally, in the area of the growth cones, an increasing activity was determined. By inhibiting voltage-dependent calcium channels, their expression and function in the differentiation process were confirmed. A comparable pattern of maturation of these channels was found in CN NSCs and primary CN neurons. The present results show that neural stem cells of the rat cochlear nucleus differentiated not only morphologically but also functionally. Spontaneous calcium activities are of great relevance in terms of neurogenesis and integration into existing neuronal structures. These functional aspects of neurogenesis within the auditory pathway could serve as future targets for the exogenous control of neuronal regeneration.
Candida albicans is ubiquitously present, and colonization in the nose and oral cavity is common. In healthy patients, it usually does not act as a pathogen, but in some cases can cause diseases. The influence of C. albicans as a trigger of T cell activation on the pathogenesis of chronic rhinosinusitis (CRS) is controversial, and its exact role is not clear to date. The aim of the present study was to detect and characterize C. albicans-specific CD4+ and CD8+ T cells in patients with CRS, with and without nasal polyps. Tissue and blood samples were collected from patients suffering from chronic rhinosinusitis with (CRSwNP) and without nasal polyps (CRSsNP), and from healthy controls. A peptide pool derived from C. albicans antigen was added to tissue and blood samples. After 6 days, lymphocytes were analyzed by multicolor flow cytometry. Activation was assessed by the intracellular marker Ki-67, and the cytokine secretion was measured. Tissue CD8+ T cells of CRSsNP patients showed a significantly higher proportion of Ki-67+ cells after activation with C. albicans antigen compared to peripheral blood CD8+ T cells. Cytokine secretion in response to C. albicans antigen was similar for all study groups. In this study, C. albicans-specific CD4+ and CD8+ T cells were detected in peripheral blood and mucosal tissue in all study groups. In patients suffering from CRSsNP, C. albicans-specific CD8+ T cells were relatively enriched in the nasal mucosa, suggesting that they might play a role in the pathogenesis of CRSsNP.
In deafness, which is caused by the malfunctioning of the inner ear, an implantation of a cochlear implant (CI) is able to restore hearing. The CI is a neural prosthesis that is located within the cochlea. It replaces the function of the inner hair cells by direct electrical stimulation of the auditory nerve fibers. The CI enables many deaf or severe hearing-impaired people to achieve a good speech perception. Nevertheless, there is a lot of potential for further improvements. Compared to normal-hearing listeners rate pitch discrimination is much worse. Rate pitch discrimination is the ability to distinguish the pitch of two stimuli with two different pulse rates. This ability is important for enjoying music as well as speech perception (in noise). Further, the small dynamic range in electrical hearing (compared to normal-hearing listeners) and therefore the small intensity resolution limits the performance of CI users. Both, rate pitch coding and dynamic range were investigated in this doctoral thesis.
For the first issue, a pitch discrimination task was designed to determine the just-noticeable-difference (JND) in pitch with 200 and 400 pps as reference. Additionally to the default biphasic pulse (single pulse) the experiment was performed with double pulses. The double pulse consists out of two biphasic pulses directly after each other and a small interpulse interval (IPI) in between. Three different IPIs (15, 50, and 150 µs) were tested. The statistical analysis of JNDs revealed no significant effects between stimulation with single-pulse or double-pulse trains.
A follow-up study investigated an alternating pulse train consisting of single and double pulses. To investigate if the 400 pps alternating pulse train is comparable in pitch with the 400 pps single-pulse train, a pairwise pitch comparison test was conducted. The alternating pulse train was compared with single-pulse trains at 200, 300 and 400 pps. The results showed that the alternating pulse train is for most subjects similar in pitch with the 200 pps single-pulse train. Therefore, pitch perception seemed to be dominated by the double pulses within the pulse train.
Accordingly, double pulses with different amplitudes were tested. Based on the facilitation effect, a larger neuronal response was expected by stimulating with two pulses with a short IPI within the temporal facilitation range. In other studies, this effect was shown to be maximal in CIs of the manufacturer Cochlear, with first pulse amplitudes set at or slightly below the electrically evoked compound action potential (ECAP) threshold. The second pulse amplitude did not influence the facilitation effect and therefore could be choose at will. Similarly, this effect was tested in this thesis with CIs of the manufacturer MED-EL. Nevertheless, to achieve a proper signal-to-noise ratio, technical issues had to be addressed like a high noise floor, resulting in incorrect determination of the ECAP threshold. After solving this issues, the maximum facilitation effect was around the ECAP threshold as in the previous study with Cochlear. For future studies this effect could be used in a modified double pulse rate pitch experiment with the first pulse amplitude at ECAP threshold and the second pulse amplitude variable to set the most comfortable loudness level (MCL).
The last study within this thesis investigated the loudness perception at two different loudness levels and the resulting dynamic range for different interphase-gaps (IPG). A larger IPG can reduce the amplitude at same loudness level to save battery power. However, it was unknown if the IPG has an influence on the dynamic range. Different IPGs (10 and 30 µs) were compared with the default IPG (2.1 µs) in a loudness matching experiment. The experiment was performed at the most comfortable loudness level (MCL) of the subject and the amplitude of half the dynamic range (50%-ADR). An upper dynamic range was calculated from the results of MCL and 50%-ADR (therefore not the whole dynamic range was covered). As expected from previous studies a larger IPG resulted in smaller amplitudes. However, the observed effect was larger at MCL than at 50%-ADR which resulted in a smaller upper dynamic range. This is the first time a decrease of this dynamic range was shown.
Einleitung: Die sog. Direct-Drive-Simulation (DDS) bietet Patienten präoperativ eine Klangsimulation des "Vibrant Soundbridge®-Hörens". Ein Floating-Mass-Transducer (FMT) wird auf das Trommelfell oder die rekonstruierte Paukenabdeckung aufgesetzt, worüber den Patienten Testsignale dargeboten werden. Ziel der Arbeit war die Evaluation einer Weiterentwicklung des Tests hin zur DDS-Tonaudiometrie und DDS-Sprachaudiometrie. Bei Bestätigung der ersten Hypothese, soll darüber hinaus geprüft werden, ob eine Hirnstammaudiometrie (BERA) über den DDS durchgeführt werden kann. Ziel der vorliegenden Arbeit ist es Referenzdaten an einer Population Normalhörender zu generieren. Zusammenfassung: Sowohl die DDS-Ton- und Sprachaudiometrie als auch die Ableitung einer BERA über den DDS-FMT sind möglich. Mit der DDS-BERA besteht nun erstmals die Möglichkeit eines objektiven DDS-Tests präoperativ. Die notwendige Einschätzung der Ankopplungsqualität zur korrekten Interpretation der DDS-BERA lässt sich elegant mit Hilfe der DDS-Tonaudiometrie ermitteln.
Objectives: The purpose of this study was to investigate the development of lateralization skills in children who received bilateral cochlear implants (CIs) in sequential operations. Methods: The lateralization skills of 9 children with a mean age of 4.1 years at the first surgery and 5.5 years at the second surgery were assessed at 3 time intervals. Children were assessed with a 3-loudspeaker setup (front, left and right) at 0.9 years (interval I) and 1.6 years (interval II) after the second implantation, and after 5.3 years of bilateral implant use (interval III) with a 9-loudspeaker setup in the frontal horizontal plane between -90° and 90° azimuth. Results: With bilateral implants, a significant decrease in lateralization error was noted between test interval I (45.0°) and II (23.3°), with a subsequent significant decrease at test interval III (4.7°). Unilateral performance with the CI did not improve significantly between the first 2 intervals; however, there was a bias of responses towards the unilateral side by test interval III. Conclusions: The lateralization abilities of children with bilateral CIs develop in a relatively short period of time (1-2 years) after the second implant. Children appear to be able to acquire binaural skills after bilateral cochlear implantation.
Auditory Brainstem Implantation Improves Speech Recognition in Neurofibromatosis Type II Patients

This prospective study aimed to determine speech understanding in neurofibromatosis type II (NF2) patients following implantation of a MED-EL COMBI 40+ auditory brainstem implant (ABI). Patients (n = 32) were enrolled postsurgically. Nonauditory side effects were evaluated at fitting and audiological performance was determined using the Sound Effects Recognition Test (SERT), Monosyllable-Trochee-Polysyllable (MTP) test and open-set sentence tests. Subjective benefits were determined by questionnaire. ABI activation was documented in 27 patients, 2 patients were too ill for testing and 3 patients were without any auditory perception. SERT and MTP outcomes under auditory-only conditions improved significantly between first fitting and 12-month follow-up. Open-set sentence recognition improved from 5% at first fitting to 37% after 12 months. The number of active electrodes had no significant effect on performance. All questionnaire respondents were ‘satisfied' to ‘very satisfied' with their ABI. An ABI is an effective treatment option in NF2 patients with the potential to provide open-set speech recognition and subjective benefits. To our knowledge, the data presented herein is exceptional in terms of the open-set speech perception achieved in NF2 patients.
Objectives: To assess the subjective and objective performance of the new fine structure processing strategy (FSP) compared to the previous generation coding strategies CIS+ and HDCIS. Methods: Forty-six adults with a minimum of 6 months of cochlear implant experience were included. CIS+, HDCIS and FSP were compared in speech perception tests in noise, pitch scaling and questionnaires. The randomized tests were performed acutely (interval 1) and again after 3 months of FSP experience (interval 3). The subjective evaluation included questionnaire 1 at intervals 1 and 3, and questionnaire 2 at interval 2, 1 month after interval 1. Results: Comparison between FSP and CIS+ showed that FSP performed at least as well as CIS+ in all speech perception tests, and outperformed CIS+ in vowel and monosyllabic word discrimination. Comparison between FSP and HDCIS showed that both performed equally well in all speech perception tests. Pitch scaling showed that FSP performed at least as well as HDCIS. With FSP, sound quality was at least as good and often better than with HDCIS. Conclusions: Results indicate that FSP performs better than CIS+ in vowel and monosyllabic word understanding. Subjective evaluation demonstrates strong user preferences for FSP when listening to speech and music.
Objective: To evaluate the safety and effectiveness of round window (RW), oval window (OW), CliP and Bell couplers for use with an active middle ear implant. Methods: This is a multicenter, long-term, prospective trial with consecutive enrollment, involving 6 university hospitals in Germany. Bone conduction, air conduction, implant-aided warble-tone thresholds and Freiburger monosyllable word recognition scores were compared with unaided preimplantation results in 28 moderate-to-profound hearing-impaired patients after 12 months of follow-up. All patients had previously undergone failed reconstruction surgeries (up to 5 or more). In a subset of patients, additional speech tests at 12 months postoperatively were used to compare the aided with the unaided condition after implantation with the processor switched off. An established quality-of-life questionnaire for hearing aids was used to determine patient satisfaction. Results: Postoperative bone conduction remained stable. Mean functional gain for all couplers was 37 dB HL (RW = 42 dB, OW = 35 dB, Bell = 38 dB, CliP = 27 dB). The mean postoperative Freiburger monosyllable score was 71% at 65 dB SPL. The postimplantation mean SRT<sub>50</sub> (speech reception in quiet for 50% understanding of words in sentences) improved on average by 23 dB over unaided testing and signal-to-noise ratios also improved in all patients. The International Outcome Inventory for Hearing Aids (IOI-HA)quality-of-life questionnaire was scored very positively by all patients. Conclusion: A significant improvement was seen with all couplers, and patients were satisfied with the device at 12 months postoperatively. These results demonstrate that an active implant is an advantage in achieving good hearing benefit in patients with prior failed reconstruction surgery.
Introduction: Since 1996, the preferred approach for positioning the active middle-ear implant Vibrant Soundbridge© is a mastoidectomy and a posterior tympanotomy. With this device, placement of the floating mass transducer (FMT) on the long incus process is the standard method for treatment of mild-to-severe sensorineural hearing loss in the case of normal middle-ear anatomy. The aim of this study was to determine the vibrational effectiveness of FMT placement at the short incus process. Materials and Methods: An extended antrotomy and a posterior tympanotomy were performed in 5 fresh human temporal bones. As a control for normal middle-ear function, the tympanic membrane was stimulated acoustically and the vibration of the stapes footplate and the round-window (RW) membrane were (sequentially) measured by laser Doppler vibrometry. Vibration responses for coupling of an FMT to the long incus process (standard coupling) were compared to those for coupling to the short incus process. Results: Apart from narrow frequency bands near 3 and 9 kHz for the stapes footplate and RW membrane, respectively, the velocity responses presented no significant differences between standard coupling of the FMT and coupling to the short incus process. Conclusion: Coupling the FMT to the short incus process may be a viable alternative in cases where the surgical approach is limited to an extended antrotomy. A reliable technique for attachment to the short incus process has yet to be developed.
Neural stem cells (NSCs) have been recently identified in the inferior colliculus (IC). These cells are of particular interest, as no casual therapeutic options for impaired neural structures exist. This research project aims to evaluate the neurogenic potential in the rat IC from early postnatal days until adulthood. The IC of rats from postnatal day 6 up to 48 was examined by neurosphere assays and histological sections. In free-floating IC cell cultures, neurospheres formed from animals from early postnatal to adulthood. The amount of generated neurospheres decreased in older ages and increased with the number of cell line passages. Cells in the neurospheres and the histological sections stained positively with NSC markers (Doublecortin, Sox-2, Musashi-1, Nestin, and Atoh1). Dissociated single cells from the neurospheres differentiated and were stained positively for the neural lineage markers β-III-tubulin, glial fibrillary acidic protein, and myelin basic protein. In addition, NSC markers (Doublecortin, Sox-2, CDK5R1, and Ascl-1) were investigated by qRT-PCR. In conclusion, a neurogenic potential in the rat IC was detected and evaluated from early postnatal days until adulthood. The identification of NSCs in the rat IC and their age-specific characteristics contribute to a better understanding of the development and the plasticity of the auditory pathway and might be activated for therapeutic use.
Das Ziel der vorliegenden Arbeit war die Untersuchung einer Kohorte erwachsener Patienten mit einseitiger Taubheit (single-sided deafness SSD), die ein Cochlea-Implantat erhalten haben. Es sollten Faktoren ermittelt werden, die es ermöglichen, eine Voraussage über die spätere Hörleistung mit Cochlea Implantat treffen zu können.
Der Beobachtungszeitraum umfasste die Jahre 2009 - 2016. Das Patientenkollektiv bestand aus 55 einseitig ertaubten Erwachsenen. Die Sprachverständlichkeit wurde mit dem Freiburger Zahlentest bei 65 dB SPL, dem Freiburger Einsilbertest bei 65 dB SPL und 80 dB SPL, sowie dem HSM-Satztest ohne Störgeräusch bei 65 dB SPL getestet. Es hat sich gezeigt, dass mit steigender Hörerfahrung mit einer Verbesserung der Hörkompetenz zu rechnen ist. Im untersuchten Patientenkollektiv schnitten Patienten mit der Kombination aus einer langen Ertaubungsdauer und einer entzündlichen Ätiologie signifikant schlechter ab als die Vergleichsgruppe.
Purpose
In cancer of unknown primary (CUP), positron emission tomography/computed tomography (PET/CT) with the glucose analog [\(^{18}\)F]FDG represents the standard imaging approach for localization of the malignant primary. Frequently, however, [\(^{18}\)F]FDG PET/CT cannot precisely distinguish between small occult tumors and chronic inflammation, especially in Waldeyer’s tonsillar ring. To improve the accuracy for detecting primary tumors in the Waldeyer’s tonsillar ring, the novel PET tracer [\(^{68}\)Ga]Ga-FAPI-4 for specific imaging of fibroblast activation protein (FAP) expression was used as a more specific target for cancer imaging.
Methods
Eight patients with suspicion of a malignant tumor in Waldeyer’s tonsillar ring or a CUP syndrome were examined. PET/CT scans with [\(^{18}\)F]-FDG and [\(^{68}\)Ga]Ga-FAPI-4 were performed for pre-operative tumor localization. After surgical resection, histopathological and immunohistochemical results were compared to PET/CT findings.
Results
Histopathology revealed a palatine or lingual tonsil carcinoma in all patients. In case of lymph node metastases smaller than 7 mm in size, the [\(^{18}\)F]FDG PET/CT detection rate of cervical lymph node metastases was higher than that of [\(^{68}\)Ga]FAPI PET/CT, while both tracers identified the primary tumors in all eight cases. The size of the primary and the lymph node metastases was directly correlated to the respective FAP expression, as detected by immunohistochemistry. The mean SUVmax for the primary tumors was 21.29 ± 7.97 for \(^{18}\)F-FDG and 16.06 ± 6.29 for \(^{68}\)Ga-FAPI, respectively (p = 0.2). The mean SUVmax for the healthy contralateral tonsils was 8.38 ± 2.45 for [\(^{18}\)F]FDG and 3.55 ± 0.47 for [\(^{68}\)Ga]FAPI (p < 0.001). The SUVmax ratio of [68Ga]FAPI was significantly different from [\(^{18}\)F] FDG (p = 0.03). Mean TBRmax for the [\(^{68}\)Ga]Ga-FAPI-4 tracer was markedly higher in comparison to [\(^{18}\)F]FDG (10.90 vs. 4.11).
Conclusion
Non-invasive imaging of FAP expression by [\(^{68}\)Ga]FAPI PET/CT resulted in a better visual detection of the malignant primary in CUP, as compared to [\(^{18}\)F]FDG imaging. However, the detection rate of lymph node metastases was inferior, presumably due to low FAP expression in small metastases. Nevertheless, by offering a detection method for primary tumors with the potential of lower false positive rates and thus avoiding biopsies, patients with CUP syndrome may benefit from [\(^{68}\)Ga]FAPI PET/CT imaging.
The five tubulin-binding cofactors (TBC) are involved in tubulin synthesis and the formation of microtubules. Their importance is highlighted by various diseases and syndromes caused by dysfunction or mutation of these proteins. Posttranslational modifications (PTMs) of tubulin promote different characteristics, including stability-creating subpopulations of tubulin. Cell- and time-specific distribution of PTMs has only been investigated in the organ of Corti in gerbils. The aim of the presented study was to investigate the cell type-specific and time-specific expression patterns of TBC proteins and PTMs for the first time in murine cochleae over several developmental stages. For this, murine cochleae were investigated at the postnatal (P) age P1, P7 and P14 by immunofluorescence analysis. The investigations revealed several profound interspecies differences in the distribution of PTMs between gerbil and mouse. Furthermore, this is the first study to describe the spatio-temporal distribution of TBCs in any tissue ever showing a volatile pattern of expression. The expression analysis of TBC proteins and PTMs of tubulin reveals that these proteins play a role in the physiological development of the cochlea and might be essential for hearing.
Objective
Cochlear implantation has become a well-accepted treatment option for people with single-sided deafness (SSD) and has become a clinical standard in many countries. A cochlear implant (CI) is the only device which restores binaural hearing. The effect of microphone directionality (MD) settings has been investigated in other CI indication groups, but its impact on speech perception in noise has not been established in CI users with SSD. The focus of this investigation was, therefore, to assess binaural hearing effects using different MD settings in CI users with SSD.
Methods
Twenty-nine experienced CI users with SSD were recruited to determine speech reception thresholds with varying target and noise sources to define binaural effects (head shadow, squelch, summation, and spatial release from masking), sound localization, and sound quality using the SSQ12 and HISQUI19 questionnaires. Outcome measures included the MD settings “natural”, “adaptive”, and “omnidirectional”.
Results
The 29 participants involved in the study were divided into two groups: 11 SONNET users and 18 OPUS 2/RONDO users. In both groups, a significant head shadow effect of 7.4–9.2 dB was achieved with the CI. The MD setting “adaptive” provided a significant head shadow effect of 9.2 dB, a squelch effect of 0.9 dB, and spatial release from masking of 7.6 dB in the SONNET group. No significant summation effect could be determined in either group with CI. Outcomes with the omnidirectional setting were not significantly different between groups. For both groups, localization improved significantly when the CI was activated and was best when the omnidirectional setting was used. The groups’ sound quality scores did not significantly differ.
Conclusions
Adaptive directional microphone settings improve speech perception and binaural hearing abilities in CI users with SSD. Binaural effect measures are valuable to quantify the benefit of CI use, especially in this indication group.
Die chronische Rhinosinusitis (CRS) ist eine entzündliche Erkrankung der Nase und Nasennebenhöhlen mit einem Bestehen von Symptomen über zwölf Wochen im Jahr. In Europa sind über 10% der erwachsenen Bevölkerung von ihr betroffen und leiden unter einer stark verminderten Lebensqualität. Nach dem Phänotyp lassen sich zwei Formen unterscheiden: die chronische Rhinosinusitis mit Polypen (CRScNP) und ohne Polypen (CRSsNP). Meist wird unter europäischen Probanden bei der CRScNP eine hauptsächlich eosinophile Th2- vermittelte und bei der CRSsNP ein gemischtes Profil aus Th1/Th17/Th2-vermittelter Entzündung beschrieben. Doch dies erfasst nicht die Vielfalt dieser Erkrankung. Erst kürzlich konnten Endotypen, basierend auf der Zytokinsekretion, identifiziert werden, die auf ein multifaktorielles Geschehen schließen lassen. Nachdem im Nasensekret von über 90% der CRS Patienten Pilze nachgewiesen werden konnten erscheint es wichtig, deren Rolle in der Pathogenese der CRS zu ergründen. Besonders interessant sind dabei Aspergillus fumigatus und Candida albicans. Als opportunistische Pilze könnten sie im Rahmen der CRS in der Lage sein bei einigen Endotypen durch eine verstärkte Aktivierung von Immunzellen die lokale Entzündungsreaktion aufrecht zu erhalten. Es wurden lokale und periphere CD4+ und CD8+ T-Zellen von Patienten mit CRScNP, CRSsNP und einer gesunden Kontrollgruppe isoliert. Eine Kultivierung mit den Antigenen von C. albicans und A. fumigatus erfolgte in einer Konzentration von 1 µg/ml über 6 Tage. Im Anschluss daran wurden die Zellen für das Durchflusszytometer angefärbt und ihre Aktivität über die Ki67+ Expression gemessen. In den Überständen der Kultivierung wurde durch das Durchflusszytometer mittels Multiplexassay die produzierten Zytokinkonzentrationen der Zellen bestimmt. In dieser Arbeit konnte eine Stimulierung der lokalen und peripheren T-Zellen durch die beiden Antigene gezeigt werden...
Leukoplakien der Stimmlippe können verschiedene histopathologische Diagnosen wie beispielsweise Hyper- oder Parakeratosen, dysplastische Prozesse oder invasive Karzinome des Epithels zugrunde liegen.
Bisher werden klinisch detektierte Leukoplakien nach Probeexzision und histologischer Aufarbeitung anhand verschiedener histomorphologischer Klassifikationssysteme, wie zum Beispiel dem System der WHO oder dem „Ljubljana classification“-System, eingeordnet.
Hinsichtlich der Aussagekraft in Bezug auf die Prognose einer Entwicklung eines invasiven Plattenepithelkarzinoms sind die genannten Systeme jedoch eingeschränkt.
In der Raman-Spektroskopie wird das zu untersuchende Gewebe mit einem Laser bestrahlt. Die beim Durchdringen des Materials entstehende Raman-Streuung ist charakteristisch für verschiedene Moleküle und Bindungen. So kann mithilfe dieser Methodik auf die molekularbiologische Zusammensetzung des Gewebes geschlossen werden.
In dieser Arbeit wurde die Klassifikation verschiedener Dysplasiegrade des Stimmlippenepithels mittels Spektraldaten aus der Raman-Spektroskopie nachvollzogen und erörtert, ob aus den gewonnenen Informationen auf das Risiko der Entstehung eines invasiven Plattenepithelkarzinoms geschlossen werden kann.
Durch den Vergleich der Mittelwertspektren dysplasiefreier, dysplastischer und maligne entarteter Gewebeproben sowie der Methodik einer Hauptkomponentenanalyse kann auf eine unterschiedliche Zusammensetzung der Studiengruppen an Bestandteilen der DNA, Proteine, Lipide und Kohlenhydrate geschlossen werden.
Unter Verwendung einer linearen Diskriminanzanalyse wurden Modelle erstellt, um die Klassifizierbarkeit der verschiedenen Studiengruppen nachzuvollziehen. Außerdem wurden die Modelle auf die Aussagekraft einer Prognose hinsichtlich einer möglichen Karzinomentwicklung überprüft.
Durch diese Modelle können gute Differenzbarkeiten durch die Spektraldaten der einzelnen Studiengruppen untereinander erreicht werden. Zudem kann mit hoher Spezifität, Sensitivität und Genauigkeit prognostiziert werden, ob sich beim jeweiligen Patienten ein invasives Plattenepithelkarzinom der Stimmlippe entwickeln wird.
Um die erlangten Erkenntnisse zu validieren und das Verfahren in den klinischen Alltag zu integrieren, bedarf es jedoch prospektiver Studien.
The human pathogen Bordetella pertussis targets the respiratory epithelium and causes whooping cough. Its virulence factor adenylate cyclase toxin (CyaA) plays an important role in the course of infection. Previous studies on the impact of CyaA on human epithelial cells have been carried out using cell lines derived from the airways or the intestinal tract. Here, we investigated the interaction of CyaA and its enzymatically inactive but fully pore-forming toxoid CyaA-AC– with primary human airway epithelial cells (hAEC) derived from different anatomical sites (nose and tracheo-bronchial region) in two-dimensional culture conditions. To assess possible differences between the response of primary hAEC and respiratory cell lines directly, we included HBEC3-KT in our studies. In comparative analyses, we studied the impact of both the toxin and the toxoid on cell viability, intracellular cAMP concentration and IL-6 secretion. We found that the selected hAEC, which lack CD11b, were differentially susceptible to both CyaA and CyaA-AC–. HBEC3-KT appeared not to be suitable for subsequent analyses. Since the nasal epithelium first gets in contact with airborne pathogens, we further studied the effect of CyaA and its toxoid on the innate immunity of three-dimensional tissue models of the human nasal mucosa. The present study reveals first insights in toxin–cell interaction using primary hAEC.
Adipose-derived stem cells (ASCs) have been extensively studied in the field of stem cell research and possess numerous clinical applications. Cell labeling is an essential component of various experimental protocols and Hoechst 33342 (H33342) represents a cost-effective and easy methodology for live staining. The purpose of this study was to evaluate the labeling of rat ASCs with two different concentrations of H33342 (0.5 μg/mL and 5 μg/mL), with particular regard to usability, interference with cell properties, and potential DNA damage. Hoechst 33342 used at a low concentration of 0.5 μg/mL did not significantly affect cell proliferation, viability, or differentiation potential of the ASCs, nor did it cause any significant DNA damage as measured by the olive tail moment. High concentrations of 5 μg/mL H33342, however, impaired the proliferation and viability of the ASCs, and considerable DNA damage was observed. Undesirable colabeling of unlabeled cocultivated cells was seen in particular with higher concentrations of H33342, independent of varying washing procedures. Hence, H33342 labeling with lower concentrations represents a usable method, which does not affect the tested cell properties. However, the colabeling of adjacent cells is a drawback of the technique.
The goal of this doctoral thesis is to identify appropriate methods for the estimation of connectivity and for measuring synchrony between spike trains from in vitro neuronal networks. Special focus is set on the parameter optimization, the suitability for massively parallel spike trains, and the consideration of the characteristics of real
recordings. Two new methods were developed in the course of the optimization which outperformed other methods from the literature. The first method “Total spiking probability edges” (TSPE) estimates the effective connectivity of two spike trains, based on the
cross-correlation and a subsequent analysis of the cross-correlogram. In addition to the estimation of the synaptic weight, a distinction between excitatory and inhibitory connections is possible. Compared to other methods, simulated neuronal networks could be estimated with higher accuracy, while being suitable for the analysis of massively parallel spike trains. The second method “Spike-contrast” measures the synchrony of parallel spike trains
with the advantage of automatically optimizing its time scale to the data. In contrast to other methods, which also adapt to the characteristics of the data, Spike-contrast is more robust to erroneous spike trains and significantly faster for large amounts of parallel spike trains. Moreover, a synchrony curve as a function of the time scale is generated by Spike-contrast. This optimization curve is a novel feature for the analysis of parallel spike trains.
Patienten mit leicht bis hochgradigen Schallleitungs-, Schallempfindungs- und kombinierten Schwerhörigkeiten werden routinemäßig nach erfolglosem Hörgerätetrageversuch mit aktiven Mittelohrimplantaten versorgt. Aktive Mittelohrimplantate können an verschiedene Strukturen des Mittelohrs angekoppelt werden. Der Ort der Ankopplung ist abhängig vom Hörverlust und der individuellen Physiologie des Mittelohres. Die Hörverbesserung ist dabei stark von der Kopplungseffizienz des Implantatwandlers an die Mittelohrstruktur abhängig.
Aktuell gibt es keine zufriedenstellende Möglichkeit die Kopplungseffizienz intraoperativ zu bestimmen. Daher wird eine objektive Methode eingeführt, um intraoperativ auditorische Hirnstammantworten (BERAs) bei Stimulation über das Implantat abzuleiten. Die Vibrant Soundbrigde® (VSB) wird dabei mit einem Drahtlosüberträger (miniTEK, Signia GmbH, Erlangen) und der Carina®-Aktuator über ein Audiokabel mit der BERA-Anlage verbunden. Die BERA-Anlage überträgt die Stimuli direkt an das Implantat, welches an die Mittelohrstruktur angekoppelt ist. Die BERA-Antworten werden bei der VSB durch einen optimierten VSB-CE-Chirp und beim Carina®-System durch den Standard CE-Chirp evoziert, beginnend bei Pegeln oberhalb der Knochenleitungshörschwelle bis unter die Registrierungsschwelle. Diese Methode kann die intraoperative Integrität des Implantats sowie die Kopplungseffizienz bestimmen, um eine Aussage über den zu erwartenden Hörerfolg treffen zu können. Darüber hinaus kann die versorgte Hörschwelle verwendet werden, um die Anpassung bei Kindern oder schwierigen Fällen zu unterstützen und um eine Hörverschlechterung über die Zeit zu erfassen.
Zusammenfassend, konnte eine Methode zur Bestimmung der intraoperativen Kopplungseffizienz während der Implantation von VSBs und Carinas® etabliert werden. Darüber hinaus werden intraoperative BERA-Daten von 30 VSB- und 10-Carina®-Patienten sowie deren Hörergebnisse gezeigt.
Pulssynchrone Ohrgeräusche – Befunderhebung bei einer standardisiert untersuchten Patientengruppe

Die Arbeit umfasst die Auswertung der klinischen Daten von 108 konsekutiv am Kopf- und Schädelbasis-Zentrum, Klinikum Fulda, zwischen Juli 2013 und September 2015 nachuntersuchten Patienten mit pulssynchronen Ohrgeräuschen. Die Patienten waren gemäß eines einheitlichen, strukturierten und routinemäßigen Diagnose- und Behandlungspfads untersucht worden. Die notwendige Bildgebung umfasste die Computertomographie, die Magnetresonanztomographie sowie die Magnetresonanzangiographie und die Sonographie der Halsgefäße. Eine digitale Subtraktionsangiographie wurde nach strenger Indikationsstellung in ausgewählten Fällen vorgenommen. Ziel war es, zugrundeliegende Ursachen und Diagnosen für das Symptom pulssynchrones Ohrgeräusch festzumachen. Die erhobenen Befunde wurden je nach Plausibilitätsgrad in 4 Gruppen klassifiziert: eindeutig, wahrscheinlich, plausibel, und unklar. Zusätzlich erfolgte die Auswertung der angewandten therapeutischen Behandlungsansätze und ihrer Ergebnisse.
Die häufigsten Ursachen waren ein hochstehender Bulbus der Vena jugularis interna mit 19%, eine Schleife der Arteria cerebelli inferior anterior im inneren Gehörgang mit 15% gefolgt von einer arteriovenösen Fistel mit 10% und Gefäßstenosen mit 9%. Bei ca. 30% unseres Patientenkollektivs konnte keine Quelle für das pulsierende Ohrgeräusch eruiert werden. Bei 29% der Patienten konnte die Ursache als eindeutig klassifiziert werden, bei 25% als plausible Ursache und bei 17% als wahrscheinlich. Bei den insgesamt 14 Patienten, die entweder eine konservative oder eine interventionell/chirurgische Behandlung erhielten, kam es bei 71% zu einer Beseitigung des Ohrgeräusches. 5 von 7 Patienten mit einer a.v. Fistel waren, nach einer erfolgreichen endovaskulären Intervention, postoperativ beschwerdefrei. Eine angioplastische Intervention mittels Stenting führte bei zwei Patienten mit einer ACI Stenose (>70%) zur Beseitigung des Ohrgeräusches. Jeweils ein Patient profitierte von der Behandlung einer Bogengangsdehiszenz, einer intrakraniellen Hypertension sowie einer chirurgischen Sanierung eines Paraganglioms, nach vorgegangener endovaskulärer Embolisation, mit der daraus resultierenden Auslöschung des Ohrgeräusches. Bei den restlichen Patienten persistierte es trotz Behandlung.
Ein pulssynchrones Ohrgeräusch hat viele äußerst unterschiedliche Ursachen. Für eine sachgerechte Evaluation und adäquate Behandlung von Patienten mit pulssynchronem Ohrgeräusch ist eine enge multidisziplinäre Zusammenarbeit unabdingbar. Eine eindeutige Diagnose kann oft nur gestellt werden, wenn alle klinischen sowie alle bildmorphologischen Befunde, die auf einem einheitlichen, routinemäßigen Untersuchungsprotokoll basieren, zusammengetragen und kritisch abgestimmt werden.
Trotz hoher Erfolgsrate und niedriger Komplikationsrate der arteriographischen Embolisation bei der Nasenblutung kommt diese Behandlungsmethode in vielen Institutionen nicht als Standard-Maßnahme im klinischen Kontext zum Einsatz und wird nur als alternativ im Falle eines Scheiterns der chirurgischen arteriellen Clippung in der Blutstillung eingesetzt.
In der vorliegenden retrospektiven Arbeit wurden am Kopf- und Schädelbasiszentrum des Klinikums Fulda die Effizienz und die Sicherheit der perkutanen Embolisation der A. sphenopalatina mit Polyvinylalkohol-Partikeln (150-250 µm) in der Behandlung, der mit konservativen Maßnahmen refraktären Epistaxis untersucht.
Methode: Retrospektive Analyse von 99 Patienten, die von Januar 2001 bis Dezember 2018 bei refraktärer, idiopathisch klassifizierter Epistaxis durch eine arteriographische endovaskuläre Embolisation behandelt wurden.
Ergebnisse: Es wurden innerhalb des Berichtszeitraums insgesamt 100 Embolisationen bei Epistaxis durchgeführt. Nach Ausschluss von 2 Prozeduren mit bekannten Blutungsursachen wurden 98 Embolisationen bei 95 Patienten mit idiopathischer Epistaxis ausgewertet. Trotz technisch erfolgreicher Intervention in 95% der Fälle, war die Blutung in 81,6% der Fälle zu stillen. Leichte Komplikationen traten in 5% der Fälle auf. Schwere Komplikationen dagegen wurden in keinem einzigen Fall erfasst.
Schlussfolgerung: Das Ergebnis dieser Untersuchung bestätigt die Rolle der angiografischen Embolisation der A. sphenopalatina sowohl als einer erfolgreichen, als auch als einer sicheren Methode in der Behandlung der konservativ refraktären Epistaxis.
Aufgrund der geringen Invasivität, hohen Erfolgsrate und niedrigen Komplikationsrate sollte die transkutane Embolisation als Standard-Therapie nach dem Scheitern der konservativen Maßnahmen zur Blutstillung einer Epistaxis zum Einsatz kommen. Die chirurgische Blutstillung durch die arterielle Clippung sollte nur im Falle eines Misserfolgs der Embolisation als Verfahren der zweiten Wahl bei der Blutstillung eingesetzt werden.
Sprachaudiometrische Tests sind subjektive Testverfahren um die Sprachverständlichkeit zu messen. Sie dienen unter anderem zur Indikation für Hörhilfen, zur Verlaufskontrolle und zur Begutachtung. Der deutschsprachige Sprachraum ist von den Freiburger Satztests geprägt. Dennoch gibt es im deutschsprachigen Raum weitere sprachaudiometrische Testverfahren. Der Oldenburger Satztest ist ein moderner Sprachtests, der als Matrixtest syntaktisch korrekte und semantisch inkorrekte Sätze als Sprachmaterial nutzt.
Diese Dissertation untersucht altersabhängige Lerneffekte an einem normalhörenden Probandenkollektiv. Dazu wurden 40 Probanden in homogenen Altersgruppen auf den bereits in der Literatur vielfach beschriebenen Lerneffekt untersucht. Die Dissertation konnte zeigen, dass sich der Lerneffekt in allen Altersgruppen ähnlich verhält. Die Lerneffekte sind damit nicht altersabhängig. Der Lerneffekt zeigte in allen Altersgruppen signifikante Verbesserungen in der fünften durchgeführten Testliste im Vergleich zur ersten durchgeführten Testliste.
Bei Tumoren von Kopf und Hals kann primär oder adjuvant durch Bestrahlung therapiert werden. Die Folgen dieser Behandlung können Xerostomie, Karies, Infektionen, Dysphagie oder Mundgeruch sein. Diese Nebenwirkungen vermindern die Lebensqualität des Patienten. Unterschiedliche Behandlungsansätze haben aufgrund von therapiebedingten Einschränkungen nicht den Weg in den klinischen Alltag gefunden. Eine Alternative zu den vorhandenen Behandlungsansätzen kann das Tissue Engineering sein. Das Ziel einer Normalisierung der Speichelproduktion nach Behandlung soll durch eine implantierbare, künstliche Speicheldrüse erreicht werden. Kann humanes natives Speicheldrüsengewebe der Parotis auf gradientenfreiem dreidimensional aufgebauten Polyurethan wachsen und seine Funktionalität beibehalten?
Humane Parotiszellen wurden von 20 Patienten im Alter von 42 - 90 Jahren durch Operation entnommenen und in Polystyrol-Zellkulturflaschen mit dem Nährmedium BEGM herangezüchtet. Es erfolgte eine 2D-Zellverteilung der reinen Parotiskultur. Zur Kontrolle der Vitalität zwischen den Passagen wurde eine Trypan-Blau Färbung verwendet. Als Trägermaterial der Zellen wurde eine biokompatible, abbaubare Matrix aus ε-Polycaprolacton verarbeitet. Die Übertragung der humanen Parotiszellen wurde mit einer Kleberproteinlösung, bestehend aus den Hauptbestandteilen Aprotinin, Fibrinogen und der Thrombinlösung durchgeführt. 7,14 und 21 Tage nach Aufbringung wurde der Überstand der zeitgleich entnommenen Konstrukte zur Überprüfung des α-Amylase konserviert. Zusätzlich wurden an den 3 Untersuchungstagen Konstrukte für die Anfertigung von histologischen Schnitten, quantitativer PCR, indirekter Immunfluoreszenz und zur Elektronenmikroskopie entnommen. Zur Überprüfung der Funktionalität der angezüchteten Speicheldrüsenzellen wurde das Enzym α-Amylase und das Wasserkanalprotein Aquaporin 5 herangezogen.
Bei der Kultivierung der humanen Speicheldrüsenzellen konnte durch den Vitalitätstest Trypan-Blau Färbung in Kombination mit einer Neubauerzählkammer eine konstant hohe Anzahl an vitalen Zellen bis zur 4. Passage nachgewiesen werden. Durch die Lebend/Tot Färbung auf FDA/EB Basis der Konstrukte über die Untersuchungszeit von 14 Tagen konnte keine Vermehrung von avitalen Zellen mikroskopisch festgestellt werden. Die statistische Auswertung mittels Boxplots des ELISA berechnete für den ersten Untersuchungstag einen Median auf niedrigem Niveau von 4,4 U/l und sank im weiteren Zeitverlauf am Untersuchungstag 21 auf die niedrigsten Median von 2,2 U/l ab. α-Amylase konnte an allen 3 Tagen mittels quantitativer PCR und indirekter Immunfluoreszenz belegt werden. Aquaporin 5 als Funktionsnachweis war in der vorliegenden Studie nicht signifikant durch quantitative PCR beweisbar. Die Rasterelektronenmikroskopie bildete adhärente Zellen in kugeliger Form aus den besiedelten Matrices nach 7 Tagen Kultivierung ab. Durch die Transmissionselektronenmikroskopie konnten Zellen, die Zellfortsätze ausgebildet hatten nach 14 Tagen beobachtet werden. Der Versuch, histologische Schnitte auf Grundlage der Paraffineinbettung oder Kryo-Konservierung zu erzeugen, musste frustran abgeschlossen werden.
Eine Kultivierung von Speicheldrüsenzellen auf einer Matrix aus ε-Polycaprolacton ohne Gradienten ist eingeschränkt umsetzbar. Die Studie konnte zeigen, dass das Wachstum der Zellen auf konstant niedrigem Niveau über den Untersuchungszeitraum von 21 Tagen lag. Der Funktionsnachweis von α-Amylase auf absinkendem niedrigem Niveau sowie fehlender Bestätigung von Aquaporin 5 kann als stationäre Phase des Wachstums interpretiert werden. Zur Verbesserung der Zellentwicklung sollte die besiedelte Matrix zu einem 3D-Zellwachstum anregen. Bei sequenziell entstehender Polarität der Zellen käme es zu einer Verbesserung der Vitalität sowie der vermehrten Ausbildung von α-Amylase und Aquaporin 5. Dies könnte in einer Kombination der Zellkultur aus Parotiszellen mit Kokulturen aus humanen Myoepithelzellen und Parenchymzellen erreicht werden. Sehr gute Ergebnisse des Zellwachstums und der Zellfunktion konnten aktuell in anderen Studien auf der Trägersubstanz Matrigel oder durch Rebesiedelung von dezellularisierten Organen beobachtet werden.
Objectives: The aim of this work is to define critical warning brainstem auditory evoked potential (BAEP) signs as a marker for the postoperative hearing outcome.
Study design: Retrospective study
Setting: Tertiary referral center
Patients: 162 patients who underwent resection of acoustic neuroma via a transtemporal approach with intraoperative monitoring (IOM) at the Department of Otorhinolaryngology, Plastic, Esthetic and Reconstructive Head and Neck Surgery, from January 2011 to December 2017.
Interventions: BAEP was performed in all patients; while intraoperative direct recording of the cochlear nerve function was done in 131 patients.
Main Outcome Measure: postoperative hearing thresholds (Pure tone audiometry).
Results: The most significant risk factor is the permanent loss of wave V as it increases the risk of postoperative hearing loss by 18 times; followed by three-steps increment of the stimulus intensity as it increases the risk by 5.75 times; and finally the response thresholds obtained during the intraoperative direct recording of cochlear nerve function. Each unite increment of the threshold increases the risk of postoperative hearing loss by 6.7%.
Conclusions: We believe that the intraoperative BAEP critical signs during IOM detected in this study can be used as a helpful tool to predict postoperative hearing loss in patients with acoustic neuroma.
Die E-Zigarette gewinnt in den letzten Jahren immer mehr an Popularität. Die Frage der Toxizität ist jedoch noch nicht abschließend geklärt, und es besteht weltweite Unsicherheit bei der Verwendung der E-Zigarette.
Die vorliegende Arbeit untersucht menschliche Nasenschleimhautzellen nach Dampfexposition mit Propylenglykol, einem Hauptbestandteil der Liquide, auf mögliche akute Entzündungsreaktionen, zytotoxische und genotoxische Wirkungen.
Die Nasenschleimhautzellen von 10 Probanden wurden im Air-Liquid-Interface kultiviert und anschließend verschiedenen Konzentrationen von Propylenglykol ausgesetzt. Die Analyse erfolgte unter Verwendung eines Trypanblau-Tests, eines Comet-Assays, eines Mikrokern-Tests und eines IL-6- und IL-8-Sandwich-ELISAs.
Der Trypanblau-Test zeigte keine Reduktion der Vitalität. Im Sandwich-ELISA konnte kein Anstieg der IL-6- und IL-8-Konzentrationen festgestellt werden. Im Comet-Assay zeigte das Olive Tail Moment in allen untersuchten Konzentrationen eine Schädigung im Vergleich zur Negativkontrolle. Es zeigte sich auch eine dosisabhängige Schädigung. Ein Unterschied zwischen der Reinsubstanz und der Negativkontrolle konnte im Mikrokern-Test festgestellt werden.
Es wurden reparierbare Schäden im Comet-Assay gefunden. Im Mikrokern-Test konnten diese nur in der Reinsubstanzkonzentration bestätigt werden. Die E-Zigarette sollte restriktiv verwendet werden, bis Langzeitstudien vorliegen. Darüber hinaus sollten die Hersteller die Inhaltsstoffe der Flüssigkeiten eindeutig angeben.
Proliferationsverhalten epithelialer Tumorzellen in Gegenwart von humanen mesenchymalen Stammzellen

Die Stammzellforschung spielt eine zentrale Rolle in der Optimierung der zielgerichteten und selektiven Tumortherapie durch den angenommenen Tumortropismus von mesenchymalen Stammzellen. Bisher konnten keine eindeutigen Verhaltensmuster mesenchymaler Stammzellen in Gegenwart von Tumorzellen verzeichnet werden. Es gibt sowohl proliferationsfördernde als auch -hemmende Einflüsse. Mögliche Gründe für diese divergierenden Ergebnisse in Bezug auf die Tumorproliferation, im Speziellen auf die Kopf-Hals-Tumorzelllinie FaD, aufzudecken, war das Ziel dieser Arbeit.
We review the role of oscillations in the brain and in the auditory system showing that the ability of humans to distinguish changes in pitch can be explained as a precise analysis of temporal information in auditory signals by neural oscillations. The connections between auditory brain stem chopper neurons construct neural oscillators, which discharge spikes at various constant intervals that are integer multiples of 0.4 ms, contributing to the temporal processing of auditory cochlear output. This is subsequently spatially mapped in the inferior colliculus. Electrophysiological measurements of auditory chopper neurons in different species show oscillations with periods which are integer multiples of 0.4 ms. The constant intervals of 0.4 ms can be attributed to the smallest synaptic delay between interconnected simulated chopper neurons. We also note the patterns of similarities between microcircuits in the brain stem and other parts of the brain (e.g., the pallidum, reticular formation, locus coeruleus, oculomotor nuclei, limbic system, amygdala, hippocampus, basal ganglia and substantia nigra), dedicated to the processing of temporal information. Similarities in microcircuits across the brain reflect the importance of one of the key mechanisms in the information processing in the brain, namely the temporal coupling of different neural events via coincidence detection.
Zinc oxide nanoparticles (ZnO-NPs) are commonly used for industrial applications. Consequently, there is increasing exposure of humans to them. The in vitro analysis of cytotoxicity and genotoxicity is commonly performed under standard cell culture conditions. Thus, the question arises of how the results of genotoxicity and cytotoxicity experiments would alter if human plasma was used instead of cell culture medium containing of fetal calf serum (FCS). Human mesenchymal stem cells (hMSCs) were cultured in human plasma and exposed to ZnO-NPs. A cultivation in expansion medium made of DMEM consisting 10% FCS (DMEM-EM) served as control. Genotoxic and cytotoxic effects were evaluated with the comet and MTT assay, respectively. hMSC differentiation capacity and ZnO-NP disposition were evaluated by histology and transmission electron microscopy (TEM). The protein concentration and the amount of soluble Zn2+ were measured. The cultivation of hMSCs in plasma leads to an attenuation of genotoxic and cytotoxic effects of ZnO-NPs compared to control. The differentiation capacity of hMSCs was not altered. The TEM showed ZnO-NP persistence in cytoplasm in both groups. The concentrations of protein and Zn2+ were higher in plasma than in DMEM-EM. In conclusion, the cultivation of hMSCs in plasma compared to DMEM-EM leads to an attenuation of cytotoxicity and genotoxicity in vitro.
Die fünf Tubulin-bindenden Kofaktoren (TBC) sind an der Tubulinsynthese und der Bildung von Mikrotubuli beteiligt. Ihre Bedeutung wird durch verschiedene Krankheiten und Syndrome hervorgehoben, die durch Funktionsstörungen oder Mutationen dieser Proteine verursacht werden. Posttranslationale Modifikationen (PTMs) von Tubulin fördern verschiedene Eigenschaften, einschließlich stabilitätsfördernder Subpopulationen von Tubulin. Die zell- und zeitspezifische Verteilung der PTMs ist bisher nur im Corti-Organ bei Gerbils untersucht worden. Ziel der vorliegenden Studie war es, die zelltyp- und zeitspezifischen Expressionsmuster von TBC-Proteinen und PTMs erstmals in der murinen Cochlea über mehrere Entwicklungsstadien hinweg zu untersuchen. Dazu wurden murine Cochleae im postnatalen (P) Alter P1, P7 und P14 mittels Immunfluoreszenzanalyse untersucht. Die Untersuchungen zeigten mehrere erhebliche Interspezies-Unterschiede in der Verteilung der PTMs zwischen Gerbil und Maus. Darüber hinaus ist dies die erste Studie, die die räumlich-zeitliche Verteilung von TBCs in einem Gewebe beschreibt, das ein volatiles Expressionsmuster aufweist. Die Expressionsanalyse von TBC-Proteinen und PTMs des Tubulins zeigt, dass diese Proteine eine wichtige Rolle bei der physiologischen Entwicklung der Cochlea spielen und für das Hören essentiell sein könnten.
Adenotonsillectomies are commonly performed procedures and sleep‐disordered breathing is becoming increasingly important as an indication for surgery. Because of the higher risks in patients with obstructive sleep apnoea, the required level of postoperative care for these patients is currently under discussion, and better identification of patients at risk may reduce unnecessary postoperative monitoring. To evaluate the influence of obstructive sleep apnoea, and other risk factors, on peri‐operative complications in children requiring adenotonsillectomy, we performed a retrospective case‐control study that included 1995 patients treated between January 2009 and June 2017. In our analysis, young age (OR 3.8, 95%CI 2.1–7.1), low body weight (OR 2.6, 95%CI 1.5–4.4), obstructive sleep apnoea (OR 2.4, 95%CI 1.5–3.8), pre‐existing craniofacial or syndromal disorders (OR 2.3, 95%CI 1.4–3.8) and adenotonsillectomy, compared with adenoidectomy alone, (OR 7.9, 95%CI 4.7–13.1) were identified as risk factors for complications during or after surgery, p < 0.001. All 13 patients suffering from complications more than 3 h postoperatively had obstructive sleep apnoea plus at least one more of these risk factors. Patients at risk of postoperative complications can therefore be identified by several criteria pre‐operatively, and should be monitored postoperatively using pulse oximetry overnight. For all other patients, postoperative observation on a surgical ward without extra monitoring is sufficient. Admission to paediatric intensive care should be reserved for patients suffering serious intra‐operative complications.
For all sensory organs, the establishment of spatial and temporal cortical resolution is assumed to be initiated by the first sensory experience and a BDNF-dependent increase in intracortical inhibition. To address the potential of cortical BDNF for sound processing, we used mice with a conditional deletion of BDNF in which Cre expression was under the control of the Pax2 or TrkC promoter. BDNF deletion profiles between these mice differ in the organ of Corti (BDNF \(^{Pax2}\) -KO) versus the auditory cortex and hippocampus (BDNF \(^{TrkC}\) -KO). We demonstrate that BDNF \(^{Pax2}\) -KO but not BDNF \(^{TrkC}\) -KO mice exhibit reduced sound-evoked suprathreshold ABR waves at the level of the auditory nerve (wave I) and inferior colliculus (IC) (wave IV), indicating that BDNF in lower brain regions but not in the auditory cortex improves sound sensitivity during hearing onset. Extracellular recording of IC neurons of BDNF \(^{Pax2}\) mutant mice revealed that the reduced sensitivity of auditory fibers in these mice went hand in hand with elevated thresholds, reduced dynamic range, prolonged latency, and increased inhibitory strength in IC neurons. Reduced parvalbumin-positive contacts were found in the ascending auditory circuit, including the auditory cortex and hippocampus of BDNF \(^{Pax2}\) -KO, but not of BDNF \(^{TrkC}\) -KO mice. Also, BDNF \(^{Pax2}\) -WT but not BDNF \(^{Pax2}\) -KO mice did lose basal inhibitory strength in IC neurons after acoustic trauma. These findings suggest that BDNF in the lower parts of the auditory system drives auditory fidelity along the entire ascending pathway up to the cortex by increasing inhibitory strength in behaviorally relevant frequency regions. Fidelity and inhibitory strength can be lost following auditory nerve injury leading to diminished sensory outcome and increased central noise.
Hintergrund.
Sprachaudiometrische Tests quantifizieren das Sprachverständnis. Sie dienen u.a. der Indikationsstellung, Anpassung und Kontrolle von Hörhilfen. Laut der 2017 aktualisierten Richtlinie des gemeinsamen Bundesausschusses über die Verordnung von Hilfsmitteln in der vertragsärztlichen Versorgung kann der Oldenburger Satztest für die Indikationsstellung bei der Hörgeräteversorgung eingesetzt werden, stellt aber keine zwingende Voraussetzung dar.
Ziel der Arbeit (Fragestellung).
Das Ziel der vorliegenden Arbeit stellte das strukturierte Erfassen des kurz- und langfristigen Lerneffekts bei der mehrfachen Durchführung des Oldenburger Satztests dar.
Material und Methoden. Der Oldenburger Satztest wurde 32 jungen, normalhörenden Probanden in 3 Testsitzungen à 5 Testdurchläufe à 30 Sätze im Abstand von je 50 Tagen präsentiert. Untersucht wurden der SRT-50% und der ableitbare Lerneffekt in Abhängigkeit von Geschlecht, Testsitzung, Testdurchlauf, Testmodus (offen versus geschlossen durch Vorlage einer Karte mit dem gesamten Wortrepertoire) und im geschlossenen Testdesign die Leserichtung.
Ergebnisse.
Testsitzung und Testdurchlauf konnten als signifikante Einflussfaktoren bestätigt werden. Bei der Auswertung der SRT-Werte zeigte sich ein signifikanter Lerneffekt von insgesamt 4,13 dB (2,61 dB innerhalb der ersten, 1,77 dB innerhalb der zweiten und 1,65 dB innerhalb der dritten Testsitzung). Eine Sättigung des Lerneffekts konnte nicht beobachtet werden. Einhergehend mit der Verbesserung zeigte sich eine zunehmend schnellere Antwortzeit.
Diskussion.
Eine vollständige Klärung der dem Lerneffekt zugrunde liegenden Mechanismen kann nicht gegeben werden. Die Resultate bestätigen den kurzfristigen Lerneffekt und zeigen eindeutig einen langfristigen Lerneffekt, der in Klinik und Forschung beachtet werden sollte.
Die objektive Überprüfung von tragbaren Hörsystemen bei Kleinkindern und unkooperativen Patienten ist aktuell nicht verfügbar. ASSR- und BERA-Messungen im Freifeld können Grundlage für einen Lösungsansatz sein. Es wurden in dieser Dissertationsarbeit BERA- und ASSR-Messungen im Freifeld an 20 normalhörenden Probanden durchgeführt. Die Normalhörigkeit wurde im Vorfeld durch ein Tonaudiogramm sichergestellt. Die Ergebnisse wurden mit Normwerten und den Ergebnissen aus Standardverfahren verglichen, um eine Gleichwertigkeit der verschiedenen Methoden (Einsteckhörer gegenüber Freifeld) zu überprüfen. Mit Hilfe von Voruntersuchungen wurde ein reproduzierbares Mess-Setup im Freifeld erstellt. Es wurde die räumliche Anordnung, die Voreinstellung der Messeinheit (Eclipse-System, Firma Interacoustics, Dänemark) und der Messablauf festgelegt. Zur Validierung des Mess-Setups wurden Untersuchungen im Freifeld zur Pegeleinstellung, zum Latenzverhalten der Welle V und zu den akustischen Stimuli Klick und CE-Chirp® durchgeführt. Es wurden BERA- und ASSR-Messreihen im festgelegten Mess-Setup erstellt. Vorangehend wurde bei 60 dB nHL eine Klick-BERA im Freifeld durchgeführt. Anschließend erfolgten BERA-Messungen mit NB CE-Chirps® bei 40 dB nHL und 60 dB nHL je bei den Frequenzen 500 Hz, 1 kHz, 2 kHz, 4 kHz und wurden mit Standardwerten der Eclipse für E-A-RTONE™ Einsteckhörer (Firma Etymotic Research, USA) verglichen. Als Vergleichswert wurde die Latenz der Welle V betrachtet. Hierbei ergaben sich keine signifikanten Unterschiede zwischen den beiden Gruppen. ASSR-Messungen wurden bei den Frequenzen 500 Hz, 1 kHz, 2 kHz, 4 kHz von 20 dB nHL absteigend bis zur Hörschwelle im Freifeld durchgeführt. Die ermittelten Hörschwellen wurden mit den Hörschwellen der subjektiven Tonaudiogramme verglichen. Hierbei zeigten sich keine signifikanten Unterschiede. Die Hörschwellen aus den ASSR-Messungen im Freifeld waren bei hohen Frequenzen tendenziell besser. Die Ergebnisse zeigen, dass unter den festgelegten Messbedingungen kein signifikanter Unterschied zwischen ASSR- und BERA-Messungen im Freifeld gegenüber Einsteckhörer besteht. Dies kann Grundlage für weitere Untersuchungen zur objektiven Hörgerät-Überprüfung sein.
Einleitung: Zinkoxid (ZnO) ist eines der am häufigsten für industrielle Produktionen und Konsumgüter eingesetzten Nanomaterialien. Zytotoxizität von ZnO-Nanopartikeln (NP) war bereits Gegenstand einiger Studien, jedoch sind die molekularen Schädigungsmechanismen nicht gänzlich geklärt. Über genotoxische Eigenschaften, die bereits bei sub-zytotoxischen Dosen auftreten können, ist im Allgemeinen weniger bekannt. Ziel dieser Studie war die Erstellung eines umfassenden Toxizitätsprofils von ZnO-NP durch Einsatz multipler Testsysteme.
Methoden: Neben der Plattenepithelkarzinom-Zelllinie FaDu wurden humane mesenchymale Knochenmarkstammzellen (BMSC) für unterschiedliche Zeiträume und Konzentrationen mit ZnO-NP behandelt. Zytotoxizität, Apoptoseinduktion und Zellzyklusalteration wurden durch MTT-Test, PCR und Durchflusszytometrie analysiert. Mit dem fpg-modifizierten Comet Assay wurden der Einfluss von oxidativem Stress auf das Gesamtausmaß der DNA-Schädigung untersucht.
Ergebnisse: ZnO-NP führten im MTT-Test ab 8 µg/ml zu einer Reduktion der Vitalität in FaDu-Zellen. Durchflusszytometrisch wurden eine dosis- und zeitabhängige Zunahme von Apoptose sowie Veränderungen des Zellzyklus nachgewiesen. Im Comet Assay konnte nach Inkubation mit 5 µg/ml ZnO-NP eine signifikante DNA-Fragmentierung in BMSC nachgewiesen werden. Bei allen getesteten Konzentrationen wurde oxidativer Stress als wichtiger Einflussfaktor der Schädigung nachgewiesen.
Diskussion: Vorliegende Studie liefert Hinweise dafür, dass ZnO-NP toxisch sind. Gegenwärtig ist eine definitive Aussage über das schädigende Potenzial von NP nicht zu treffen, da der Vergleich verschiedener Studien kaum möglich ist. Gerade die Verwendung von ZnO-NP als Bestandteil von Kosmetikprodukten, die repetitiv in geringen Mengen von Verbrauchern appliziert werden, sollte jedoch kritisch betrachtet werden.
Radioresistance is an important cause of head and neck cancer therapy failure. Zinc oxide nanoparticles (ZnO-NP) mediate tumor-selective toxic effects. The aim of this study was to evaluate the potential for radiosensitization of ZnO-NP. The dose-dependent cytotoxicity of ZnO-NP\(_{20 nm}\) and ZnO-NP\(_{100 nm}\) was investigated in FaDu and primary fibroblasts (FB) by an MTT assay. The clonogenic survival assay was used to evaluate the effects of ZnO-NP alone and in combination with irradiation on FB and FaDu. A formamidopyrimidine-DNA glycosylase (FPG)-modified single-cell microgel electrophoresis (comet) assay was applied to detect oxidative DNA damage in FB as a function of ZnO-NP and irradiation exposure. A significantly increased cytotoxicity after FaDu exposure to ZnO-NP\(_{20 nm}\) or ZnO-NP\(_{100 nm}\) was observed in a concentration of 10 µg/mL or 1 µg/mL respectively in 30 µg/mL of ZnO-NP\(_{20 nm}\) or 20 µg/mL of ZnO-NP\(_{100 nm}\) in FB. The addition of 1, 5, or 10 µg/mL ZnO-NP\(_{20 nm}\) or ZnO-NP\(_{100 nm}\) significantly reduced the clonogenic survival of FaDu after irradiation. The sub-cytotoxic dosage of ZnO-NP\(_{100 nm}\) increased the oxidative DNA damage compared to the irradiated control. This effect was not significant for ZnO-NP\(_{20 nm}\). ZnO-NP showed radiosensitizing properties in the sub-cytotoxic dosage. At least for the ZnO-NP\(_{100 nm}\), an increased level of oxidative stress is a possible mechanism of the radiosensitizing effect.
Durch ihren Tumortropismus haben mesenchymale Stammzellen (MSCs) das Interesse der onkologischen Forschung geweckt. Sie werden als potenzielles Vehikel für die zielgerichtete Tumortherapie diskutiert. Ihre Wirkung auf Tumore ist jedoch nach wie vor unklar: Es werden sowohl tumorfördernde als auch tumorhemmende Eigenschaften in der Literatur beschrieben. In der vorliegenden Arbeit wurde der Einfluss von nativen und andifferenzierten MSCs aus dem Knochenmark auf Proliferation und Vitalität von Kopf-Hals-Tumorzellen in vitro systematisch untersucht.
Entsprechend der Ergebnisse des durchgeführten Proliferationsassay und des Dot Blot Assay muss von einer protumorigenen Wirkung der MSCs auf HNSCC ausgegangen werden. Mit Hilfe von ELISA und Western Blot konnte gezeigt werden, dass der IL-6 vermittelte Aktivierung von ERK1/2 und STAT3 eine wichtige Rolle in der Interaktion zwischen MSC und HNSCC zukommt.
Angesichts dieser Ergebnisse müssen hinsichtlich eines Einsatzes von MSC in der Tumortherapie Bedenken geäußert werden. Weitere Untersuchungen zum besseren Verständnis der Interaktion sind notwendig.
Der Verlust des Hörvermögens kann im auditorischen System auf vielen verschiedenen Ebenen zu morphologischen und funktionellen Veränderungen führen. In der vorliegenden Arbeit wurden in einem Tiermodell für prälinguale Ertaubung die morphologischen Auswirkungen frühzeitiger und kompletter bilateraler Langzeitertaubung im medialen Kern des Trapezkörpers (MNTB) und im lateralen Kern des Trapezkörpers (LNTB) der mongolischen Wüstenrennmaus (Meriones unguiculatus) untersucht. Beide Kerne zählen zu den Kernen des oberen Olivenkomplexes. Hierzu werden außerdem die Kerne MSO (mediale obere Olive) und LSO (laterale obere Olive) gezählt. Die MSO ist für die Verarbeitung von interauralen Laufzeitdifferenzen (interaural time delays, ITD) zuständig, wohingegen die LSO vor allem für die Verarbeitung von interauralen Pegelunterschieden (interaural level delays, ILD) verantwortlich ist. Beide Kerne sind somit für die Lokalisation einer Schallquelle in der Horizontalebene verantwortlich. Ihnen vorgeschaltet sind die Kerne MNTB und LNTB, weshalb auch diese beiden Kerne für die Verarbeitung von ILDs und ITDs und somit für das Richtungshören eine große Rolle spielen. In dieser Studie wurden die Kerne MNTB und LNTB von insgesamt 23 mongolischen Wüstenrennmäusen in der Nissl-Färbung und den immunhistologischen Färbungen GABA und Glycin lichtmikroskopisch untersucht. Zwölf der Versuchstiere wurden durch die Gabe einer ototoxischen Lösung, bestehend aus dem Aminoglykosid Kanamycin und dem Schleifendiuretikum Bumetanid, ertaubt. Beim MNTB wurden die angefertigten Hirnschnitte bezüglich der Querschnittsflächen des Kerngebiets, der Zellzahlen, -größen und -dichte in der Nissl-Färbung untersucht, beim LNTB fand die Auswertung der vier Parameter in allen drei Färbungen statt. Weiterhin sollte ein Nachweis struktureller Unterschiede entlang tonotoper Gradienten erfolgen sowie die Überprüfung der Kerngebiete auf Hemisphärenunterschiede. Die Ergebnisse unserer Untersuchungen zum MNTB zeigten bei ertaubten Tieren weder eine Abnahme der Querschnittsfläche, noch eine Abnahme der Zellzahl oder -dichte. Es wurde lediglich eine signifikante Abnahme der Größe glycinerger Neurone bei ertaubten Tieren über den gesamten MNTB festgestellt. Die Abnahme der Neuronengröße könnte die Verfügbarkeit von Glycin beeinflussen und somit die Balance zwischen Exzitation und Inhibition stören. Nachfolgende Kerne wie MSO, LSO und LNTB können hierdurch in ihrer Funktion, der Schalllokalisation und dem Sprachverständnis im Rauschen, beeinträchtigt werden. Unsere Ergebnisse zum LNTB zeigten bezüglich Kernquerschnittsfläche, Zellzahl und -dichte, entsprechend dem MNTB, keine signifikanten Unterschiede zwischen den Tieren der Kontrollgruppe und den ertaubten Tieren. Im Gegensatz zum MNTB konnte allerdings eine ertaubungsbedingte Abnahme der Zellgröße im gesamten LNTB nicht nachgewiesen werden. Es ist anzuführen, dass unsere Ergebnisse zur LNTB-Zellgröße und -zahl aufgrund der kleinen Anzahl verwertbarer Zellen in den Schnitten der immunhistochemischen Färbungen Glycin und GABA nur bedingt aussagekräftig sind. Die ertaubungsbedingten morphologischen Veränderungen der Kerne MNTB und LNTB sind im Wesentlichen vergleichbar mit den Auswirkungen eines partiellen Hörverlusts im Alter (Presbyakusis). Unterschiede bezüglich der Hemisphären konnten nicht nachgewiesen werden. Beim MNTB bestehen Gradienten der Zellgrößen mit großen Zellen in den lateralen, tieffrequenten Bereichen und kleinen Zellen in den medialen, hochfrequenten Bereichen. Die Anordnung der Zellgrößen entlang der tonotopen Gradienten scheint auch nach frühzeitiger Ertaubung erhalten zu bleiben. Bezüglich eines möglichen Gradienten der Zellgröße im LNTB ist auf Grund der teilweise sehr geringen Anzahl an auswertbaren Zellen keine abschließende Beurteilung möglich.
Tinnitus is the perception of a phantom sound that affects between 10 and 15% of the general population. Despite this considerable prevalence, treatments for tinnitus are presently lacking. Tinnitus exhibits a diverse array of recognized risk factors and extreme clinical heterogeneity. Furthermore, it can involve an unknown number of auditory and non-auditory networks and molecular pathways. This complex combination has hampered advancements in the field. The identification of specific genetic factors has been at the forefront of several research investigations in the past decade. Nine studies have examined genes in a case-control association approach. Recently, a genome-wide association study has highlighted several potentially significant pathways that are implicated in tinnitus. Two twin studies have calculated a moderate heritability for tinnitus and disclosed a greater concordance rate in monozygotic twins compared to dizygotic twins. Despite the more recent data alluding to genetic factors in tinnitus, a strong association with any specific genetic locus is lacking and a genetic study with sufficient statistical power has yet to be designed. Future research endeavors must overcome the many inherent limitations in previous study designs. This review summarizes the previously embarked upon tinnitus genetic investigations and summarizes the hurdles that have been encountered. The identification of candidate genes responsible for tinnitus may afford gene based diagnostic approaches, effective therapy development, and personalized therapeutic intervention.
Background
Fabry disease (FD) is an X-linked recessive hereditary lysosomal storage disorder which results in the accumulation of globotriaosylceramid (Gb3) in tissues of kidney and heart as well as central and peripheral nervous system.
Besides prominent renal and cardiac organ involvement, cochlear symptoms like high-frequency hearing loss and tinnitus are frequently found with yet no comprehensive data available in the literature.
Objective
To examine hearing loss in patients with FD depending on cardiac and renal function.
Material and methods
Single-center study with 68 FD patients enrolled between 2012 and 2016 at the Department of Oto-Rhino-Laryngology, Plastic, Aesthetic and Reconstructive Head and Neck Surgery of the University of Würzburg. Every subject underwent an oto-rhino-laryngological examination as well as behavioral, electrophysiological and electroacoustical audiological testing. High-frequency thresholds were evaluated by using a modified PTA\(_{6}\) (0.5, 1, 2, 4, 6, 8) and HF-PTA (6, 8 kHz). Renal function was measured by eGFR, cardiac impairment was graduated by NYHA class.
Results
Sensorineural hearing loss was detected in 58.8% of the cohort, which occurred typically in sudden episodes and affected especially high frequencies. Hearing loss is asymmetric, beginning unilaterally and affecting the contralateral ear later. Tinnitus was reported by 41.2%. Renal and cardiac impairment influenced the severity of hearing loss (p < 0.05).
Conclusions
High frequency hearing loss is a common problem in patients with FD. Although not life-threatening, it can seriously reduce quality of life and should be taken into account in diagnosis and therapy. Optimized extensive hearing assessment including higher frequency thresholds should be used.
Background: Zinc oxide nanoparticles (ZnO NPs) are among the most frequently applied nanomaterials in consumer products. Evidence exists regarding the cytotoxic effects of ZnO NPs in mammalian cells; however, knowledge about the potential genotoxicity of ZnO NPs is rare, and results presented in the current literature are inconsistent. Objectives: The aim of this review is to summarize the existing data regarding the DNA damage that ZnO NPs induce, and focus on the possible molecular mechanisms underlying genotoxic events. Methods: Electronic literature databases were systematically searched for studies that report on the genotoxicity of ZnO NPs. Results: Several methods and different endpoints demonstrate the genotoxic potential of ZnO NPs. Most publications describe in vitro assessments of the oxidative DNA damage triggered by dissoluted Zn2+ ions. Most genotoxicological investigations of ZnO NPs address acute exposure situations. Conclusion: Existing evidence indicates that ZnO NPs possibly have the potential to damage DNA. However, there is a lack of long-term exposure experiments that clarify the intracellular bioaccumulation of ZnO NPs and the possible mechanisms of DNA repair and cell survival.
Zinc oxide nanoparticles (ZnO-NP) are widely spread in consumer products. Data about the toxicological characteristics of ZnO-NP is still under controversial discussion. The human skin is the most important organ concerning ZnO-NP exposure. Intact skin was demonstrated to be a sufficient barrier against NPs; however, defect skin may allow NP contact to proliferating cells. Within these cells, stem cells are the most important toxicological target for NPs. The aim of this study was to evaluate the genotoxic and cytotoxic effects of ZnO-NP at low-dose concentrations after long-term and repetitive exposure to human mesenchymal stem cells (hMSC). Cytotoxic effects of ZnO-NP were measured by the 3-(4,5-dimethylthiazol-2-yl)-2,5-diphenyl tetrazolium bromide (MTT) assay. Furthermore, genotoxicity was evaluated by the comet assay. For long-term observation over 6 weeks, transmission electron microscopy (TEM) was applied. The results of the study indicated cytotoxic effects of ZnO-NP beginning at high concentrations of 50 μg/mL and genotoxic effects in hMSC exposed to 1 and 10 μg/mL ZnO-NP. Repetitive exposure enhanced cyto- but not genotoxicity. Intracellular NP accumulation was observed up to 6 weeks. The results suggest cytotoxic and genotoxic potential of ZnO-NP. Even low doses of ZnO-NP may induce toxic effects as a result of repetitive exposure and long-term cellular accumulation. This data should be considered before using ZnO-NP on damaged skin.
Otitis media (OM) is a common pediatric disease for which systemic antibiotics are often prescribed. While local treatment would avoid the systemic treatment side-effects, the tympanic membrane (TM) represents an impenetrable barrier unless surgically breached. We hypothesized that the TM might harbor innate biological mechanisms that could mediate trans-TM transport. We used two M13-bacteriophage display biopanning strategies to search for mediators of trans-TM transport. First, aliquots of linear phage library displaying 10\(^{10th}\) 12mer peptides were applied on the TM of rats with active bacterial OM. The middle ear (ME) contents were then harvested, amplified and the preparation re-applied for additional rounds. Second, the same naïve library was sequentially screened for phage exhibiting TM binding, internalization and then transit. Results revealed a novel set of peptides that transit across the TM to the ME in a time and temperature dependent manner. The peptides with highest transport capacities shared sequence similarities. Historically, the TM was viewed as an impermeable barrier. However, our studies reveal that it is possible to translocate peptide-linked small particles across the TM. This is the first comprehensive biopanning for the isolation of TM transiting peptidic ligands. The identified mechanism offers a new drug delivery platform into the ME.
Hintergrund: Die Studienlage zu Kautabak und Zigarettenrauch ist eindeutig und zeigt karzinogenes Potential. Über Schnupftabak ist hingegen wenig bekannt, vor allem auf zellulärer Ebene gibt es keine ausreichenden wissenschaftlichen Publikationen. Somit lässt sich die eventuell mutagene Wirkung von Schnupftabak nur schwer einschätzen. In Konsequenz stützt sich die WHO in ihrer Einstufung des Schnupftabaks als nicht karzinogen auf eine sehr eingeschränkte Datenlage.
Ziel: Ziel der vorliegenden Arbeit war es, Schnupftabak auf mögliche zyto- und genotoxische Effekte auf humane Lymphozyten und Nasenschleimhautzellen zu untersuchen um ggf. tumorinitiierende Effekte darzustellen.
Material und Methoden: Es kam eine Schnupftabaksorte ohne Menthol und eine Sorte mit Mentholzusatz zum EInsatz. Die benötigten Nasenschleimhautzellen und Lymphozyten wurden von 10 Probanden gewonnen und eine Stunde lang mit einem Schnupftabak-DMSO-Gemisch (2000µg/ml bis 0,01µg/ml) inkubiert. Zur Analyse wurde der Trypanblautest, dee Comet Assay und der Mikrokerntest verwendet.
Ergebnis: Der Trypanblautest zeigte keinen Abfall der Vitalität. Beim Comet Assay ergab sich bei Lymphozyten ein signifikanter Anstieg der DNA-Fragmentierung ab 100µg/ml, bei Nasenschleimhautzellen ab 1000µg/ml. Der Mikrokerntest wies keine signifikante Zunahme der Mikrokerne auf. Es konnte kein Unterschied zwischen den beiden Tabaksorten aufgezeigt werden.
Diskussion: Es zeigte sich eine Schädigung der Erbsubstanz im Comet Assay, die möglicherweise reparabel ist. Irreparable DNA-Schäden im Sinne von Mikrokernen wurden nicht gefunden. Nach diesen Ergebnissen muss die Einstufung der WHO in Zweifel gezogen werden. Untersuchungen mit weiteren Endpunkten der Genotoxizität sind somit gerechtfertigt, um zu einer fundierten Beurteilung des Risikopotentials von Schnupftabak zu gelangen.
Shisha-Tabak benötigt im Vergleich zur Zigarette höhere Konzentrationen des Feuchthaltemittels Glycerin. Seit Mai 2016 ist die bis dahin gültige Limitierung von Feuchthaltemitteln in Tabak auf 5 % aufgehoben. Derzeit ist das toxikologische Profil des Glycerins jedoch noch nicht hinreichend erforscht. Ziel dieser Arbeit war es, Glycerin auf mögliche zyto- und genotoxische
Effekte zu untersuchen, um so das Gefährdungspotenzial durch Glycerin im Shisha-Tabak zu beurteilen und die tabakkontrollpolitische Situation in Deutschland zu diskutieren.
Dafür wurden Lymphozyten sowie Nasenschleimhautzellen von 10 Patienten für eine Stunde Glycerin (0,001 mol/l bis 6,0 mol/l) exponiert. Durch den Trypanblau-Ausschlusstest wurden die Zellen auf Zytotoxizität, mittels Einzelzellgelelektrophorese (Comet Assay) und Mikrokern-Test auf Genotoxizität untersucht.
Im Trypanblau-Ausschlusstest traten bei Lymphozyten sowie nasalen Mukosazellen signifikante Vitalitätsabfälle ab Glycerin-Konzentrationen von 1,0 mol/l auf. Im Comet Assay konnten für beide Zellgruppen signifikante Unterschiede des Olive Tail Moments (OTM) ab 1,0 mol/l nachgewiesen werden. Beim Mikrokern-Test zeigten sich keine signifikanten Zunahmen der
Mikrokern-Anzahl.
Es konnten zyto- und genotoxische Effekte ab Konzentrationen von 1,0 mol/l nachgewiesen werden. Dies überschreitet die reale Glycerin-Belastung im Hauptstromrauch der Shisha jedoch deutlich. Dennoch handelt es sich bei Genotoxizität um ein stochastisches Risiko. Ebenso sind toxische Effekte, beispielsweise durch Erhitzung, bereits bei geringeren Konzentrationen denkbar. Für eine umfangreichere Beurteilung von Feuchthaltemitteln im Shisha-Tabak sind weitere Untersuchungen indiziert. Darüber hinaus besteht enormer Handlungsbedarf zur weiteren Einführung tabakkontrollpolitischer Maßnahmen in Deutschland.
Das Polyether-Antibiotikum Salinomycin stammt ursprünglich aus der Tierzucht und wurde kürzlich als Inhibitor epithelialer Tumorstammzellen identifiziert. Vor einem möglichen Einsatz in der Onkologie müssen zunächst toxische Effekte von Salinomycin in nicht malignen humanen Zellen evaluiert werden. In dieser Arbeit sollte daher die geno- und zytotoxische Wirkung von Salinomycin auf Zellen humaner nasaler Mukosa und Lymphozyten untersucht werden.
Dazu wurden Zellen humaner nasaler Mukosa (Einzelzellkulturen und Miniorgankulturen) und Lymphozyten von 10 Patienten für 24h mit Salinomycin (0,1 bis 175 μM) inkubiert. Zur Untersuchung der Genotoxizität wurde die Einzelzellmikrogelelektrophorese angewendet. Zur Untersuchung der Zytotoxizität wurden der MTT-Test sowie die Annexin-Propidiumjodid-Durchflusszytometrie durchgeführt. Zusätzlich wurde mit einem Sandwich-ELISA die IL-8-Konzentration in den Überständen der Miniorgankulturen gemessen, um die proinflammatorische Wirkung von Salinomycin bewerten zu können.
Es zeigte sich kein signifikanter Anstieg der DNA-Schädigung der behandelten Zellen im Vergleich zur Negativkontrolle. Im MTT-Test und in der Annexin-Propidiumjodid-Durchflusszytometrie ließ sich ab Konzentrationen von 10-20 μM eine signifikante Reduktion der Vitalität der behandelten Zellen nachweisen. Der Sandwich-ELISA zeigte einen Anstieg der IL-8-Konzentration bei einer Salinomycin-Konzentration von 5 μM und 10 μM.
Diese Ergebnisse lassen darauf schließen, dass Salinomycin in den verwendeten Konzentrationen zytotoxisch und proinflammatorisch aber nicht genotoxisch wirkt. Eine toxische Wirkung auf Tumorstammzellen konnte schon ab 0,5 μM beobachtet werden. Dennoch sollten weitere Untersuchungen folgen, um den genauen Wirkmechanismus von Salinomycin zu klären und dessen zytotoxische Wirkung auf nicht maligne Zellen reduzieren zu können.
Hintergrund: Für die zeitgerechte Diagnose und Therapie einer permanenten kindlichen Hörstörung ist ein universelles Neugeborenen-Hörscreening (NHS) unverzichtbar. Der Goldstandard für das NHS basiert auf der Ableitung von akustisch evozierten Hirnstammpotentialen (Brainstem evoked response audiometry, BERA). An der Klinik und Poliklinik für Hals-, Nasen- und Ohrenkrankheiten, plastische und ästhetische Operationen der Universität Würzburg erfolgt das NHS mit dem automatischen BERA-Testsystem MAICO MB11 BERAphone® (MAICO Diagnostics GmbH). In vorausgegangenen Arbeiten zur Optimierung dieser Messmethode ergaben sich Hinweise auf eine Beziehung zwischen dem Vigilanzzustand (Wachheitsgrad) der Neugeborenen und der Reizrate.
In der vorliegenden Studie wurde untersucht, ob durch die Verwendung einer bestimmten Reizrate in Abhängigkeit vom Vigilanzzustand eine Verkürzung der Messzeit bzw. der Testdauer erreicht werden kann.
Methoden: An 72 Neugeborenen (Altersmittelwert: 2,3 Tage) wurden 409 Messungen mit dem MB11 BERAphone® im Rahmen des universellen NHS der Universitäts-HNO-Klinik Würzburg durchgeführt. Es wurden drei verschiedene Vigilanzzustände (wach, dösend, schlafend) mit den Reizraten 20/s, 40/s, 60/s, 80/s, 90/s, 100/s untersucht.
Ergebnisse: Es konnte gezeigt werden, dass die Nachweiszeit beim NHS vom Vigilanzzustand der Neugeborenen während der Untersuchung beeinflusst wurde. Bei schlafenden Neugeborenen wurden kürzere Nachweiszeiten erzielt als bei wachen Neugeborenen. In Abhängigkeit von der verwendeten Reizrate waren diese Nachweiszeitunterschiede zwischen den schlafenden und wachen Neugeborenen unterschiedlich groß. Bei einer Reizrate von 60/s war im Vergleich zu den übrigen Reizraten (20/s, 40/s, 80/s, 90/s, 100/s) kein signifikanter Unterschied zwischen den Nachweiszeiten der wachen und schlafenden Neugeborenen nachweisbar.
Fazit: Eine Reizrate von 60/s ist für das NHS vorteilhaft. Diese Stimulationsrate wird am wenigsten durch die Vigilanz der Neugeborenen beeinflusst. Es treten somit weniger vigilanzbedingte Verlängerungen der Screening-Zeit auf.

Despite recent advancements in diagnostic tools, the genomic landscape of hereditary hearing loss remains largely uncharacterized. One strategy to understand genome-wide aberrations includes the analysis of copy number variation that can be mapped using SNP-microarray technology. A growing collection of literature has begun to uncover the importance of copy number variation in hereditary hearing loss. This pilot study underpins a larger effort that involves the stage-wise analysis of hearing loss patients, many of whom have advanced to high-throughput sequencing analysis.
Data description:
Our data originate from the Infinium HumanOmni1-Quad v1.0 SNP-microarrays (Illumina) that provide useful markers for genome-wide association studies and copy number variation analysis. This dataset comprises a cohort of 108 individuals (99 with hearing loss, 9 normal hearing family members) for the purpose of understanding the genetic contribution of copy number variations to hereditary hearing loss. These anonymized SNP-microarray data have been uploaded to the NCBI Gene Expression Omnibus and are intended to benefit other investigators interested in aggregating platform-matched array patient datasets or as part of a supporting reference tool for other laboratories to better understand recurring copy number variations in other genetic disorders.
Der Hörsinn ist für uns Menschen von entscheidender Bedeutung, um mit der Umwelt kommunizieren zu können. Hörstörungen werden dabei in sensorineurale Erkrankungen der neuronalen Strukturen und nicht-sensorineurale Schallleitungsschwerhörigkeiten unterschieden.
In der vorliegenden Studie sollte es darum gehen zu untersuchen, inwiefern ein neues konditionelles Mausmodell, die Brn3.1 IRES Cre Maus, und ein bestehendes Mausmodell, die pmn-Maus, sich eignen, um die Erkrankung der auditorischen Neuropathie nachzubilden. Die Brn3.1 IRES Cre Maus wurde zur Evaluation der Expression von Cre-Rekombinase unter der Aktivität des Brn3.1 Promotors mit gefloxten Reporter-Mauslinien verkreuzt. Die pmn-Maus ist ein anerkanntes Modell einer Motoneuronerkrankung, hatte aber in vorherigen Untersuchungen erhöhte Hörschwellen gezeigt.
Alle verwendeten Mauslinien wurden mittels ABR und DPOAE frequenzspezifisch untersucht, um die Funktion der Haarzellen im Corti´schen Organ zu evaluieren. Bei der pmn-Linie wurden die audiologischen Untersuchungen wöchentlich zwischen P21 und P35 durchgeführt. Zusätzlich wurde das Corti´sche Organ zu diesen Zeitpunkten morphologisch untersucht.
Es zeigte sich, dass alle verwendeten Mauslinien unauffällige Hörschwellen verglichen zur Backgroundlinie C57BL/6 hatten sowie DPOAE-Antworten zeigten. Die Verkreuzung der Brn3.1 IRES Cre Linie mit den beiden Reporterlinien zeigte eine nachweisbare Cre-Rekombinase-Expression unter der Aktivität des Brn3.1 Promotors nur an den postnatalen Tagen P14 und P21. Diese Expression erfolgte mosaikartig in den äußeren Haarzellen. Mit Hilfe einer RT-PCR wurde eine Diskrepanz zwischen Genotyp und Expression des Reporterproteins im Gewebe festgestellt. Dies ließ vermuten, dass die Expression von Cre-Rekombinase durch gene silencing Prozesse unterdrückt wurde und Brn3.1 als Promotor nicht leistungsstark genug war, um eine Cre-Rekombinase-Expression steuern zu können.
Die pmn-Linie zeigte in ABR-Untersuchungen bereits zum Zeitpunkt P21 erhöhte Hörschwellen in allen untersuchten Frequenzen im Vergleich zum Wildtyp. DPOAE-Antworten waren in der pmn-Linie nur bedingt auslösbar. Es zeigte sich in der morphologischen Evaluation ein Verlust von äußeren Haarzellen über die gesamte Länge des Corti´schen Organes. Durch einen TUNEL-Assay konnte das Absterben dieser Zellen durch apoptotische Vorgänge nachgewiesen werden. Der pmn-Phänotyp entsteht durch eine Mutation im TBCE Gen. Dieses Gen kodiert für ein Protein, welches einen stabilisierenden Einfluss auf die Organisation der Mikrotubuli hat. Die TBCE-Verteilung im Corti´schen Organ zeigte, dass dieses hauptsächlich in den äußeren Haarzellen und inneren Stützzellen exprimiert wird und damit wahrscheinlich einen bedeutenden Einfluss auf die Erhaltung der Haarzellen hat. Eine Analyse des Hörnervs zeigte einen Verlust von Mikrotubuli.
Neben diesen in vivo Untersuchungen sollte außerdem eine gliazellfreie Kultur von dissoziierten auditorischen Neuronen etabliert werden. Hierfür wurden mehrere Faktoren einer Primärzellkultur in Bezug auf ihren Einfluss auf die Gesamtzellzahl, den prozentualen Neuronanteil und die Axonlänge der Neurone untersucht. Das Medium, die Beschichtung des Zellkulturgefäßes, die Gabe von Neurotrophinen/Zytokinen und der Einsatz eines Zytostatikums wurden separat untersucht. Es zeigte sich, dass das Medium, die Beschichtung und Neurotrophin-/Zytokingabe hauptsächlich einen Einfluss auf das axonale Längenwachstum von Neuronen haben. Den Prozentsatz der Neurone beeinflusste nur der Einsatz des Zytostatikums Cytosin-β-D-arabinofuranosid (AraC) signifikant. Die Ergebnisse wurden auch im Zusammenhang mit der reellen Neuronanzahl in Kultur gesehen.
Es ergab sich weiterhin eine Präferenz für DMEM- über NB-Medium sowie für zusätzliche Lamininbeschichtung, für den Einsatz des Zytokins LIF gegenüber den neurotrophen Faktoren BDNF und NT-3 und die Gabe des Zytostatikums AraC ab Tag 2 nach Ausplattierung in einer Konzentration von 5-10 µM. Ein kombinatorischer Einsatz dieser Präferenzen spiegelte in Summe die Ergebnisse der Versuchsreihen Neurotrophine/Zytokin und AraC wieder. Der Anteil der Neurone in der Kultur konnte im Durchschnitt auf 10-12 % gesteigert werden. Eine Verschiebung des Glia-/Neuronenanteils zugunsten letzterer ist vermutlich nur durch den Einsatz weiterer Faktoren oder anderer Methoden möglich.
Die untersuchten Mausmodelle zeigten auf Grund der Untersuchungsergebnisse nur teilweise Ähnlichkeiten mit der Erkrankung der auditorischen Neuropathie. Die Erkenntnisse zur pmn-Mauslinie über das frequenzspezifische Hörvermögen und die morphologische Degeneration im Corti´schen Organ im altersabhängigen Verlauf können aber hilfreich sein, um neben der motorischen auch die sensorische Degeneration dieser Pathologie besser zu verstehen. Zudem konnten auch umfassende Erkenntnisse für die Kultur von dissoziierten auditorischen Neuronen der Maus in Bezug auf verschiedene Variablen erhalten werden.
