Both nerve injury and complex regional pain syndrome (CRPS) can result in chronic pain. In traumatic neuropathy, the blood nerve barrier (BNB) shielding the nerve is impaired—partly due to dysregulated microRNAs (miRNAs). Upregulation of microRNA-21-5p (miR-21) has previously been documented in neuropathic pain, predominantly due to its proinflammatory features. However, little is known about other functions. Here, we characterized miR-21 in neuropathic pain and its impact on the BNB in a human-murine back translational approach. MiR-21 expression was elevated in plasma of patients with CRPS as well as in nerves of mice after transient and persistent nerve injury. Mice presented with BNB leakage, as well as loss of claudin-1 in both injured and spared nerves. Moreover, the putative miR-21 target RECK was decreased and downstream Mmp9 upregulated, as was Tgfb. In vitro experiments in human epithelial cells confirmed a downregulation of CLDN1 by miR-21 mimics via inhibition of the RECK/MMP9 pathway but not TGFB. Perineurial miR-21 mimic application in mice elicited mechanical hypersensitivity, while local inhibition of miR-21 after nerve injury reversed it. In summary, the data support a novel role for miR-21, independent of prior inflammation, in elicitation of pain and impairment of the BNB via RECK/MMP9.

Objective
Impairment of glycinergic neurotransmission leads to complex movement and behavioral disorders. Patients harboring glycine receptor autoantibodies suffer from stiff‐person syndrome or its severe variant progressive encephalomyelitis with rigidity and myoclonus. Enhanced receptor internalization was proposed as the common molecular mechanism upon autoantibody binding. Although functional impairment of glycine receptors following autoantibody binding has recently been investigated, it is still incompletely understood.
Methods
A cell‐based assay was used for positive sample evaluation. Glycine receptor function was assessed by electrophysiological recordings and radioligand binding assays. The in vivo passive transfer of patient autoantibodies was done using the zebrafish animal model.
Results
Glycine receptor function as assessed by glycine dose–response curves showed significantly decreased glycine potency in the presence of patient sera. Upon binding of autoantibodies from 2 patients, a decreased fraction of desensitized receptors was observed, whereas closing of the ion channel remained fast. The glycine receptor N‐terminal residues \(^{29}\)A to \(^{62}\)G were mapped as a common epitope of glycine receptor autoantibodies. An in vivo transfer into the zebrafish animal model generated a phenotype with disturbed escape behavior accompanied by a reduced number of glycine receptor clusters in the spinal cord of affected animals.
Interpretation
Autoantibodies against the extracellular domain mediate alterations of glycine receptor physiology. Moreover, our in vivo data demonstrate that the autoantibodies are a direct cause of the disease, because the transfer of human glycine receptor autoantibodies to zebrafish larvae generated impaired escape behavior in the animal model compatible with abnormal startle response in stiff‐person syndrome or progressive encephalitis with rigidity and myoclonus patients.
Glycine receptor (GlyR) autoantibodies are associated with stiff-person syndrome and the life-threatening progressive encephalomyelitis with rigidity and myoclonus in children and adults. Patient histories show variability in symptoms and responses to therapeutic treatments. A better understanding of the autoantibody pathology is required to develop improved therapeutic strategies. So far, the underlying molecular pathomechanisms include enhanced receptor internalization and direct receptor blocking altering GlyR function. A common epitope of autoantibodies against the GlyRα1 has been previously defined to residues 1A-33G at the N-terminus of the mature GlyR extracellular domain. However, if other autoantibody binding sites exist or additional GlyR residues are involved in autoantibody binding is yet unknown. The present study investigates the importance of receptor glycosylation for binding of anti-GlyR autoantibodies. The glycine receptor α1 harbors only one glycosylation site at the amino acid residue asparagine 38 localized in close vicinity to the identified common autoantibody epitope. First, non-glycosylated GlyRs were characterized using protein biochemical approaches as well as electrophysiological recordings and molecular modeling. Molecular modeling of non-glycosylated GlyRα1 did not show major structural alterations. Moreover, non-glycosylation of the GlyRα1N38Q did not prevent the receptor from surface expression. At the functional level, the non-glycosylated GlyR demonstrated reduced glycine potency, but patient GlyR autoantibodies still bound to the surface-expressed non-glycosylated receptor protein in living cells. Efficient adsorption of GlyR autoantibodies from patient samples was possible by binding to native glycosylated and non-glycosylated GlyRα1 expressed in living not fixed transfected HEK293 cells. Binding of patient-derived GlyR autoantibodies to the non-glycosylated GlyRα1 offered the possibility to use purified non-glycosylated GlyR extracellular domain constructs coated on ELISA plates and use them as a fast screening readout for the presence of GlyR autoantibodies in patient serum samples. Following successful adsorption of patient autoantibodies by GlyR ECDs, binding to primary motoneurons and transfected cells was absent. Our results indicate that the glycine receptor autoantibody binding is independent of the receptor’s glycosylation state. Purified non-glycosylated receptor domains harbouring the autoantibody epitope thus provide, an additional reliable experimental tool besides binding to native receptors in cell-based assays for detection of autoantibody presence in patient sera.
Second hit hypothesis in dystonia: Dysfunctional cross talk between neuroplasticity and environment?

One of the great mysteries in dystonia pathophysiology is the role of environmental factors in disease onset and development. Progress has been made in defining the genetic components of dystonic syndromes, still the mechanisms behind the discrepant relationship between dystonic genotype and phenotype remain largely unclear. Within this review, the preclinical and clinical evidence for environmental stressors as disease modifiers in dystonia pathogenesis are summarized and critically evaluated. The potential role of extragenetic factors is discussed in monogenic as well as adult-onset isolated dystonia. The available clinical evidence for a "second hit" is analyzed in light of the reduced penetrance of monogenic dystonic syndromes and put into context with evidence from animal and cellular models. The contradictory studies on adult-onset dystonia are discussed in detail and backed up by evidence from animal models. Taken together, there is clear evidence of a gene-environment interaction in dystonia, which should be considered in the continued quest to unravel dystonia pathophysiology.
The pathogenesis of Parkinson's disease (PD) is closely interwoven with the process of aging. Moreover, increasing evidence from human postmortem studies and from animal models for PD point towards inflammation as an additional factor in disease development. We here assessed the impact of aging and inflammation on dopaminergic neurodegeneration in the hm\(^{2}\)α-SYN-39 mouse model of PD that carries the human, A30P/A53T double-mutated α-synuclein gene. At 2–3 months of age, no significant differences were observed comparing dopaminergic neuron numbers of the substantia nigra (SN) pars compacta of hm\(^{2}\)α-SYN-39 mice with wildtype controls. At an age of 16–17 months, however, hm\(^{2}\)α-SYN-39 mice revealed a significant loss of dopaminergic SN neurons, of dopaminergic terminals in the striatum as well as a reduction of striatal dopamine levels compared to young, 2–3 months transgenic mice and compared to 16–17 months old wildtype littermates. A significant age-related correlation of infiltrating CD4+ and CD8\(^{+}\) T cell numbers with dopaminergic terminal loss of the striatum was found in hm\(^{2}\)α-SYN-39 mice, but not in wildtype controls. In the striatum of 16–17 months old wildtype mice a slightly elevated CD8\(^{+}\) T cell count and CD11b\(^{+}\) microglia cell count was observed compared to younger aged mice. Additional analyses of neuroinflammation in the nigrostriatal tract of wildtype mice did not yield any significant age-dependent changes of CD4\(^{+}\), CD8\(^{+}\) T cell and B220\(^{+}\) B cell numbers, respectively. In contrast, a significant age-dependent increase of CD8\(^{+}\) T cells, GFAP\(^{+}\) astrocytes as well as a pronounced increase of CD11b+ microglia numbers were observed in the SN of hm\(^{2}\)α-SYN-39 mice pointing towards a neuroinflammatory processes in this genetic mouse model for PD. The findings in the hm\(^{2}\)α-SYN-39 mouse model strengthen the evidence that T cell and glial cell responses are involved in the age-related neurodegeneration in PD. The slow and age-dependent progression of neurodegeneration and neuroinflammation in the hm\(^{2}\)α-SYN-39 PD rodent model underlines its translational value and makes it suitable for studying anti-inflammatory therapies.
Delayed cerebral vasospasm following subarachnoid hemorrhage (SAH) is a serious medical complication, characterized by constriction of cerebral arteries leading to varying degrees of cerebral ischemia. Numerous clinical and experimental studies have been performed in the last decades; however, the pathophysiologic mechanism of cerebral vasospasm after SAH still remains unclear. Among a variety of experimental SAH models, the double hemorrhage rat model involving direct injection of autologous arterial blood into the cisterna magna has been used most frequently for the study of delayed cerebral vasospasm following SAH in last years. Despite the simplicity of the technique, the second blood injection into the cisterna magna may result in brainstem injury leading to high mortality. Therefore, a modified double hemorrhage model of cisterna magna has been developed in rat recently. We describe here step by step the surgical technique to induce double SAH and compare the degree of vasospasm with other cisterna magna rat models using histological assessment of the diameter and cross-sectional area of the basilar artery
In dieser Arbeit sollte an weiblichen Sprague Dawley Ratten untersucht werden, ob sich durch die Antidepressiva Amitriptylin und Venlafaxin Schmerzverhalten nach Nervenläsion verringern läßt und wenn ja, welche Mechanismen dieser Wirkung zugrunde liegen. Den Tieren wurde einseitig der N. ischiadicus nach dem Nervenläsionsmodell der Chronic-Constriction-Injury (CCI) operiert. Das Schmerzverhalten der mit Antidepressiva behandelten Tiere wurde über zwei bis drei Wochen verblindet im Vergleich mit plazebobehandelten Tieren untersucht. Das Ausmaß von Hitzehyperalgesie und taktiler Allodynie wurde durch die Anwendung etablierter Testverfahren quantifiziert. Amitriptylin hatte in der Dosis von zweimal täglich 10 mg/kg KG i.p. keinen relevanten Effekt auf das Schmerzverhalten der Tiere. Eine akute Gabe von Amitriptylin bei Tieren, die zuvor über 19 Tage chronisch mit Amitriptylin behandelt worden waren, reduzierte die taktile Allodynie geringgradig und hatte keinen Einfluß auf die Hitzehyperalgesie. Venlafaxin in der Dosis von zweimal täglich 25 mg/kg KG p.o. reduzierte in einigen Teilversuchen mäßiggradig die Hitzehyperalgesie und die taktile Allodynie nach CCI. Auf diesen Ergebnissen aufbauend wurde versucht, die Wirkung der chronischen Venlafaxin-Medikation durch Kombination mit zusätzlich akut verabreichten alpha-adrenergen- bzw. µ-Opiat-Rezeptor-agonistischen Substanzen zu verstärken. Die Kombination von Venlafaxin mit dem alpha-2A-Rezeptoragonisten Clonidin ergab eine Wirkungsverstärkung in Bezug auf die Reduktion der Hitzehyperalgesie, wobei sich zusätzlich eine Eigenwirkung von Clonidin nachweisen ließ. Der µ-Opiat-Rezeptor-Agonist Morphin führte hingegen zu keiner signifikanten Wirkungsverstärkung in Kombination mit Venlafaxin. Im Anschluß an die jeweilige Testreihe wurde den Tieren Nervengewebe entnommen, welches nach immunhistochemischer Färbung für alpha-2A- und µ-Rezeptoren morphometrisch evaluiert wurde. Dies diente der Untersuchung der Hypothese, daß den o.g. Wirkungen eine Vermehrung der entsprechenden Rezeptoren bei Venlafaxin-behandelten Tieren zugrunde lag. Die chronische postoperative Gabe von Venlafaxin hatte keinen Einfluß auf die Anzahl von alpha-2A-Rezeptor-immunreaktiven Neuronen im Spinalganglion CCI-operierter Ratten. Allerdings ließ sich unter der Medikation die Immunreaktivität für alpha-2A-Rezeptoren vermehrt in großkalibrigen Spinalganglienneuronen nachweisen. Die chronische postoperative Gabe von Venlafaxin führte zudem zu einer Zunahme von µ-Opiat-Rezeptoren im ipsilateralen N. ischiadicus CCI-operierter Ratten. Im Spinalganglion ergab sich nach Venlafaxinbehandlung keine Veränderung der Anzahl der µ-Rezeptor-immunreaktiven Neurone. Allerdings konnte durch chronische Venlafaxin-Medikation eine Phänotyp-Verschiebung mit Auftreten von µ-Rezeptor-Immunreaktivität in großkalibrigen Neuronen, wie sie nach CCI ohne Venlafaxinbehandlung auftrat, verhindert werden. Die Ergebnisse der Arbeit zeigen, daß die chronische, zweimal tägliche Anwendung der Antidepressiva Amitriptylin und Venlafaxin das Schmerz-assoziierte Verhalten von Ratten nach einer peripheren Nervenläsion inkonstant und nur in unzureichendem Ausmaß beeinflussen. Unabhängig davon wurden in den immunhistochemischen Untersuchungen Veränderungen in der Verteilung µ- und alpha-adrenerger Rezeptoren in Spinalganglionzellen und Ischiasnerv beobachtet, die auf eine kontinuierliche Venlafaxin-Medikation zurückzuführen zu sein scheinen. Berücksichtigt man vor diesem Hintergrund die Tatsache, daß sich die Venlafaxinwirkung durch den alpha-2A-Agonisten Clonidin verstärken ließ, so bieten diese Zusammenhänge eine mögliche Erklärung für die zugrundeliegenden Mechanismen der Wirkungsvermittlung von Antidepressiva in der Behandlung einer schmerzhaften Mononeuropathie.
In addition to bradykinesia and tremor, patients with Parkinson’s disease (PD) are known to exhibit non-motor symptoms such as apathy and hypomimia but also impulsivity in response to dopaminergic replacement therapy. Moreover, a plethora of studies observe differences in electrocortical and autonomic responses to both visual and acoustic affective stimuli in PD subjects compared to healthy controls. This suggests that the basal ganglia (BG), as well as the hyperdirect pathway and BG thalamocortical circuits, are involved in affective processing. Recent studies have shown valence and dopamine-dependent changes in synchronization in the subthalamic nucleus (STN) in PD patients during affective tasks. This thesis investigates the role of dopamine, valence, and laterality in STN electrophysiology by analyzing event-related potentials (ERP), synchronization, and inter-hemispheric STN connectivity. STN recordings were obtained from PD patients with chronically implanted electrodes for deep brain stimulation during a passive affective picture presentation task. The STN exhibited valence-dependent ERP latencies and lateralized ‘high beta’ (28–40 Hz) event-related desynchronization. This thesis also examines the role of dopamine, valence, and laterality on STN functional connectivity with the anterior cingulate cortex (ACC) and the amygdala. The activity of these limbic structures was reconstructed using simultaneously recorded electroencephalographic signals. While the STN was found to establish early coupling with both structures, STN-ACC coupling in the ‘alpha’ range (7–11 Hz) and uncoupling in the ‘low beta’ range (14–21 Hz) were lateralized. Lateralization was also observed at the level of synchrony in both reconstructed sources and for ACC ERP amplitude, whereas dopamine modulated ERP latency in the amygdala. These results may deepen our current understanding of the STN as a limbic node within larger emotional-motor networks in the brain.
Im Zeitraum von 2004 bis 2016 erhielten an der Neurologischen Universitätsklinik Würzburg Patienten mit einer chronisch progredienten Multiplen Sklerose insgesamt 595 Injektionen von intrathekalem Triamcinolonacetonid. Diese Arbeit befasst sich mit Sicherheit, Nebenwirkungen und Wirksamkeit der intrathekalen Therapieform.
Tenderness and mechanical allodynia are key symptoms of malignant tumor, inflammation and neuropathy. The proinflammatory cytokine interleukin-6 (IL-6) is causally involved in all three pathologies. IL-6 not only regulates innate immunity and inflammation but also causes nociceptor sensitization and hyperalgesia. In general and in most cell types including immune cells and sensory neurons, IL-6 binds soluble mu receptor subunits which heteromerizes with membrane bound IL-6 signal transducer gp130. In the present study, we used a conditional knock-out strategy to investigate the importance of signal transducer gp130 expressed in C nociceptors for the generation and maintenance of mechanical hypersensitivity. Nociceptors were sensitized to mechanical stimuli by experimental tumor and this nociceptor sensitization was preserved at later stages of the pathology in control mice. However, in mice with a conditional deletion of gp130 in Nav1.8 expressing nociceptors mechanical hypersensitivity by experimental tumor, nerve injury or inflammation recovery was not preserved in the maintenance phase and nociceptors exhibited normal mechanical thresholds comparable to untreated mice. Together, the results argue for IL-6 signal transducer gp130 as an essential prerequisite in nociceptors for long-term mechanical hypersensitivity associated with cancer, inflammation and nerve injury.
The aim of the study was to record movement-related single unit activity (SUA) in the human subthalamic nucleus (STN) during a standardized motor task of the upper limb. We performed microrecordings from the motor region of the human STN and registered kinematic data in 12 patients with Parkinson’s disease (PD) undergoing deep brain stimulation surgery (seven women, mean age 62.0 ± 4.7 years) while they intraoperatively performed visually cued reach-to-grasp movements using a grip device. SUA was analyzed offline in relation to different aspects of the movement (attention, start of the movement, movement velocity, button press) in terms of firing frequency, firing pattern, and oscillation. During the reach-to-grasp movement, 75/114 isolated subthalamic neurons exhibited movement-related activity changes. The largest proportion of single units showed modulation of firing frequency during several phases of the reach and grasp (polymodal neurons, 45/114), particularly an increase of firing rate during the reaching phase of the movement, which often correlated with movement velocity. The firing pattern (bursting, irregular, or tonic) remained unchanged during movement compared to rest. Oscillatory single unit firing activity (predominantly in the theta and beta frequency) decreased with movement onset, irrespective of oscillation frequency. This study shows for the first time specific, task-related, SUA changes during the reach-to-grasp movement in humans.

The aim of the study was to assess attentional biases for negative affective stimuli that might be related to migraine triggers in migraine patients with either few or frequent migraine and healthy controls. Methods: Thirty-three subjects with frequent migraine (FM) or with less frequent episodic migraine, and 20 healthy controls conducted two emotional Stroop tasks in the interictal period. In task 1, general affective words and in task 2, pictures of affective faces (angry, neutral, happy) were used. For each task we calculated two emotional Stroop indices. Groups were compared using one-way ANOVAs. Results: The expected attentional bias in migraine patients was not found. Der essentielle Tremor (ET) ist eine der häufigsten Bewegungsstörungen, welcher lange Zeit als rein motorische Störung angesehen wurde. Aufgrund zunehmender Belege über nicht-motorisch Begleitsymptome wandelte sich dieses Bild jedoch in den letzten Jahren zunehmend. In der vorliegenden Arbeit untersuchten wir 113 Probanden aus der Allgemeinbevölkerung mit klinisch definitiven oder wahrscheinlichen ET anhand einer breiten Batterie neuro-psychologischer Testverfahren. Es gelang hierbei signifikante Unterschiede im Vergleich zu gesunden Eichstichproben im Hinblick auf neuro-psychologische Charakteristika, wie Apathie, Ängstlichkeit und exekutive Dysfunktion, sowie deren negativen Einfluss auf die Lebensqualität der Probanden darzustellen. Bisher werden im klinischen Alltag nicht-motorische Begleitphänomene beim ET nicht regelhaft erfasst; aufgrund unserer Ergebnisse und der Relevanz vor allem im Hinblick auf die Lebensqualität des Einzelnen halten wir jedoch die Erfassung und gegebenenfalls Behandlung dieser Symptome für ebenso relevant.
Different phosphoinositide 3-kinase isoforms mediate carrageenan nociception and inflammation

Phosphoinositide 3-kinases (PI3Ks) participate in signal transduction cascades that can directly activate and sensitize nociceptors and enhance pain transmission. They also play essential roles in chemotaxis and immune cell infiltration leading to inflammation. We wished to determine which PI3K isoforms were involved in each of these processes. Lightly anesthetized rats (isoflurane) were injected subcutaneously with carrageenan in their hind paws. This was preceded by a local injection of 1% DMSO vehicle or an isoform-specific antagonist to PI3K-α (compound 15-e), -β (TGX221), -δ (Cal-101), or -γ (AS252424). We measured changes in the mechanical pain threshold and spinal c-Fos expression (4 hours after injection) as indices of nociception. Paw volume, plasma extravasation (Evans blue, 0.3 hours after injection), and neutrophil (myeloperoxidase; 1 hour after injection) and macrophage (CD11b+; 4 hour after injection) infiltration into paw tissue were the measured inflammation endpoints. Only PI3K-γ antagonist before treatment reduced the carrageenan-induced pain behavior and spinal expression of c-Fos (P ≤ 0.01). In contrast, pretreatment with PI3K-α, -δ, and-γ antagonists reduced early indices of inflammation. Plasma extravasation PI3K-α (P ≤ 0.05), -δ (P ≤ 0.05), and -γ (P ≤ 0.01), early (0-2 hour) edema -α (P ≤ 0.05), -δ (P ≤ 0.001), and -γ (P ≤ 0.05), and neutrophil infiltration (all P ≤ 0.001) were all reduced compared to vehicle pretreatment. Later (2-4 hour), edema and macrophage infiltration (P ≤ 0.05) were reduced by only the PI3K-δ and -γ isoform antagonists, with the PI3K-δ antagonist having a greater effect on edema. PI3K-β antagonism was ineffective in all paradigms. These data indicate that pain and clinical inflammation are pharmacologically separable and may help to explain clinical conditions in which inflammation naturally wanes or goes into remission, but pain continues unabated.

This was preceded by a local injection of 1% DMSO vehicle or an isoform-specific antagonist to PI3K-α (compound 15-e), -β (TGX221), -δ (Cal-101), or -γ (AS252424). We measured changes in the mechanical pain threshold and spinal c-Fos expression (4 hours after injection) as indices of nociception. Paw volume, plasma extravasation (Evans blue, 0.3 hours after injection), and neutrophil (myeloperoxidase; 1 hour after injection) and macrophage (CD11b+; 4 hour after injection) infiltration into paw tissue were the measured inflammation endpoints. Only PI3K-γ antagonist before treatment reduced the carrageenan-induced pain behavior and spinal expression of c-Fos (P <= 0.01). In contrast, pretreatment with PI3K-α, -δ, and -γ antagonists reduced early indices of inflammation. Plasma extravasation PI3K-α (P <= 0.05), -δ (P <= 0.05), and -γ (P <= 0.01), early (0-2 hour) edema -α (P <= 0.05), -δ (P <= 0.001), and -γ (P <= 0.05), and neutrophil infiltration (all P <= 0.001) were all reduced compared to vehicle pretreatment. Later (2-4 hour), edema and macrophage infiltration (P <= 0.05) were reduced by only the PI3K-δ and -γ isoform antagonists, with the PI3K-δ antagonist having a greater effect on edema. PI3K-β antagonism was ineffective in all paradigms. These data indicate that pain and clinical inflammation are pharmacologically separable and may help to explain clinical conditions in which inflammation naturally wanes or goes into remission, but pain continues unabated.
Die Axone im peripheren Nerven unterliegen während elektrischer Erregung kontinuierlichen dynamischen Änderungen ihrer Membraneigenschaften. Auf ein Aktionspotential folgt zunächst die absolute und die relative Refraktärperiode, dann eine Periode der Übererregbarkeit („superexcitability“) und schliesslich die Zeit der späten Untererregbarkeit („delayed subexcitability“). Stimuliert man unmyelinisierte Nervenfasern über einen längeren Zeitraum, so kommt es zu einer kontinuierlichen Erhöhung der Reizschwelle und damit einhergehend zu einer Zunahme der Latenz (=“activity dependent slowing“). Dabei bestehen Unterschiede zwischen den einzelnen funktionellen Faserklassen. In dieser Arbeit konnte erstmals bei Ableitung von C-Fasern in C57BL/6 Mäusen in vitro gezeigt werden, dass hitzeunempfindliche CM- und CMC-Fasern während elektrischer Stimulation eine geringere Latenzzunahme erfuhren als hitzesensible CMH- und CMHC-Fasern, auch in der Fasererholung bestanden Unterschiede zwischen beiden Gruppen. Dass in diesem Porozess Ih–Kanäle die Latenzzunahme v.a. bei den hitzeunempfindlichen Fasern limitieren, konnte in Experimenten mit den Ih-Kanal-Blockern CsCl (5mM) und ZD 7288 (1-50µM) gezeigt werden. In Ableitungen von C-Fasern in Mäusen mit Inaktivierung des Gens von Nav1.8 kam es vor allem bei den hitzeempfindlichen Fasern häufiger zu Leitungsblocks, die Latenzänderungen waren geringer als bei den Wildtyp-Tieren. Ströme durch diesen Kanal scheinen einerseits für die Leitungssicherheit der Fasern eine Rolle zu spielen, andererseits scheinen sie auch über einen erhöhten Natriumeinstrom während des Aktionspotentials und damit einer stärkeren Aktivierung der Na+/K+-ATPase die Dauer der Refraktärperiode zu beeinflussen. Beide Mechanismen beeinflussen somit die Reizschwelle und damit die Erregbarkeit einer Faser. Sowohl die Kinetik von Ih als auch die von Nav1.8 wird durch Entzündungsmediatoren beeinflusst, damit werden sie zu interessanten Kandidaten bei der Sensibilisierung von Fasern im Rahmen von inflammatorischen und neuropathischen Schmerzen.
The pathophysiology of tremor in Parkinson’s disease (PD) is evolving towards a complex alteration to monoaminergic innervation, and increasing evidence suggests a key role of the locus coeruleus noradrenergic system (LC-NA). However, the difficulties in imaging LC-NA in patients challenge its direct investigation. To this end, we studied the development of tremor in a reserpinized rat model of PD, with or without a selective lesioning of LC-NA innervation with the neurotoxin DSP-4. Eight male rats (Sprague Dawley) received DSP-4 (50 mg/kg) two weeks prior to reserpine injection (10 mg/kg) (DR-group), while seven male animals received only reserpine treatment (R-group). Tremor, rigidity, hypokinesia, postural flexion and postural immobility were scored before and after 20, 40, 60, 80, 120 and 180 min of reserpine injection. Tremor was assessed visually and with accelerometers. The injection of DSP-4 induced a severe reduction in LC-NA terminal axons (DR-group: 0.024 ± 0.01 vs. R-group: 0.27 ± 0.04 axons/um\(^2\), p < 0.001) and was associated with significantly less tremor, as compared to the R-group (peak tremor score, DR-group: 0.5 ± 0.8 vs. R-group: 1.6 ± 0.5; p < 0.01). Kinematic measurement confirmed the clinical data (tremor consistency (% of tremor during 180 s recording), DR-group: 37.9 ± 35.8 vs. R-group: 69.3 ± 29.6; p < 0.05). Akinetic–rigid symptoms did not differ between the DR- and R-groups. Our results provide preliminary causal evidence for a critical role of LC-NA innervation in the development of PD tremor and foster the development of targeted therapies for PD patients.
Parkinson’s disease (PD) is among the most common neurodegenerative conditions, and it is characterized by the progressive loss of dopaminergic neurons and a great variability in clinical expression. Despite several effective medications, it still causes disability as all patients show treatment-resistant symptoms and complications.
A possible reason for this therapeutic-burden and great clinical variability lies in a probable misconception about its pathophysiology, one that focuses on neurodegeneration, while largely neglecting its functional consequences and the related compensatory changes. In this thesis, I expand on the hypothesis that some PD symptoms have a dysfunctional origin and reflect derangements of neural network dynamics, the means by which brain coordination supports any motor behaviour. In particular, I have investigated resting tremor and freezing of gait, two common symptoms with an enigmatic mechanism and suboptimal management.
In the case of tremor, I predicted a pathological change in response to dopamine loss, which included the activation of noradrenergic (NA) neurons of the locus coeruleus (LC) projecting to the cerebellum. This compensatory LC activation that supports dopaminergic neurons might indeed come at the expense of tremor development. To assess the role of LC-NA in tremor development, I recorded tremor occurrence in the reserpinized rat model of PD, one of very few showing tremor, after selective lesioning (with the neurotoxin DSP-4) of the LC-NA terminal axons. DSP-4 induced a severe reduction of LC-NA terminal axons in the cerebellar cortex and this was associated with a significant reduction in tremor development. Unlike its development, tremor frequency and the akinetic rigid signs did not differ between the groups, thus suggesting a dopaminergic dependency. These findings suggest that the LC-NA innervation of the cerebellum has a critical role for PD tremor, possibly by exerting a network effect, which gates the cerebello-thalamic-cortical circuit into pathological oscillations upon a dopaminergic loss in the basal ganglia.
In contrast, for the study of freezing of gait, I worked with human PD subjects and deep brain stimulation, a therapeutic neuromodulation device that in some prototypes also allows the recording of neural activity in freely-moving subjects. Gait freezing is a disabling PD symptom that suddenly impairs effective stepping, thus causing falls and disability. Also in this study, I hypothesized that the underlying pathophysiology may be represented by dysfunctional neural network dynamics that abruptly impair locomotor control by affecting the communication in the supraspinal locomotor network. To test this hypothesis, I investigated the coupling between the cortex and the subthalamic nucleus, two main nodes of the supraspinal locomotor network, in freely-moving subjects PD patients and also performed molecular brain imaging of striatal dopamine receptor density and kinematic measurements. I found that in PD patients, walking is associated with cortical-subthalamic stable coupling in a low-frequency band (i.e. θ-α rhythms). In contrast, these structures decoupled when gait freezing occurred in the brain hemisphere with less dopaminergic innervation. These findings suggest that freezing of gait is a “circuitopathy”, with dysfunctional cortical-subcortical communication.
Altogether the results of my experiments support the hypothesis that the pathophysiology of PD goes beyond neurodegenerative (loss-of-function) processes and that derangement of neural network dynamics coincides with some disabling PD symptoms, thus suggesting that PD can be interpreted as the combination of multiple circuitopathies.
Deep brain stimulation (DBS) of the subthalamic nucleus or the globus pallidus is an established treatment for Parkinson’s disease (PD) that yields a marked and lasting improvement of motor symptoms. Yet, DBS benefit on gait disturbances in PD is still debated and can be a source of dissatisfaction and poor quality of life. Gait disturbances in PD encompass a variety of clinical manifestations and rely on different pathophysiological bases. While gait disturbances arising years after DBS surgery can be related to disease progression, early impairment of gait may be secondary to treatable causes and benefits from DBS reprogramming. In this review, we tackle the issue of gait disturbances in PD patients with DBS by discussing their neurophysiological basis, providing a detailed clinical characterization, and proposing a pragmatic programming approach to support their management.
Aims:
Patients with Fabry disease (FD) characteristically develop peripheral neuropathy at an early age, with pain being a crucial symptom of underlying pathology. However, the diagnosis of pain is challenging due to the heterogeneous and nonspecific symptoms. Practical guidance on the diagnosis and management of pain in FD is needed.
Methods:
In 2014, experts met to discuss recent advances on this topic and update clinical guidance.
Results:
Emerging disease-specific tools, including FabryScan, Fabry-specific Pediatric Health and Pain Questionnaire, and Wurzburg Fabry Pain Questionnaire, and more general tools like the Total Symptom Score can aid diagnosis, characterization, and monitoring of pain in patients with FD. These tools can be complemented by more objective and quantifiable sensory testing. In male and female patients of any age, pain related to FD can be an early indication to start disease-specific enzyme replacement therapy before potentially irreversible organ damage to the kidneys, heart, or brain occurs.
Conclusion:
To improve treatment outcomes, pain should be diagnosed early in unrecognized or newly identified FD patients. Treatment should include: (a) enzyme replacement therapy controlling the progression of underlying pathology; (b) adjunctive, symptomatic pain management with analgesics for chronic neuropathic and acute nociceptive, and inflammatory or mixed pain; and (c) lifestyle modifications.
Differences in stem cell marker and osteopontin expression in primary and recurrent glioblastoma

Background
Despite of a multimodal approach, recurrences can hardly be prevented in glioblastoma. This may be in part due to so called glioma stem cells. However, there is no established marker to identify these stem cells.
Methods
Paired samples from glioma patients were analyzed by immunohistochemistry for expression of the following stem cell markers: CD133, Musashi, Nanog, Nestin, octamer-binding transcription factor 4 (Oct4), and sex determining region Y-box 2 (Sox2). In addition, the expression of osteopontin (OPN) was investigated. The relative number of positively stained cells was determined. By means of Kaplan–Meier analysis, a possible association with overall survival by marker expression was investigated.
Results
Sixty tissue samples from 30 patients (17 male, 13 female) were available for analysis. For Nestin, Musashi and OPN a significant increase was seen. There was also an increase (not significant) for CD133 and Oct4. Patients with mutated Isocitrate Dehydrogenase-1/2 (IDH-1/2) status had a reduced expression for CD133 and Nestin in their recurrent tumors. Significant correlations were seen for CD133 and Nanog between OPN in the primary and recurrent tumor and between CD133 and Nestin in recurrent tumors. By confocal imaging we could demonstrate a co-expression of CD133 and Nestin within recurrent glioma cells. Patients with high CD133 expression had a worse prognosis (22.6 vs 41.1 months, p = 0.013). A similar trend was seen for elevated Nestin levels (24.9 vs 41.1 months, p = 0.08).
Conclusions
Most of the evaluated markers showed an increased expression in their recurrent tumor. CD133 and Nestin were associated with survival and are candidate markers for further clinical investigation.
Repetitive transcranial magnetic stimulation (rTMS) is a non-invasive brain stimulation technique that has the potential to treat a variety of neurologic and psychiatric disorders. The extent of rTMS-induced neuroplasticity may be dependent on a subject’s brain state at the time of stimulation. Chronic low intensity rTMS (LI-rTMS) has previously been shown to induce beneficial structural and functional reorganisation within the abnormal visual circuits of ephrin-A2A5\(^{-/-}\) mice in ambient lighting. Here, we administered chronic LI-rTMS in adult ephrin-A2A5\(^{-/-}\) mice either in a dark environment or concurrently with voluntary locomotion. One day after the last stimulation session, optokinetic responses were assessed and fluorescent tracers were injected to map corticotectal and geniculocortical projections. We found that LI-rTMS in either treatment condition refined the geniculocortical map. Corticotectal projections were improved in locomotion+LI-rTMS subjects, but not in dark + LI-rTMS and sham groups. Visuomotor behaviour was not improved in any condition. Our results suggest that the beneficial reorganisation of abnormal visual circuits by rTMS can be significantly influenced by simultaneous, ambient visual input and is enhanced by concomitant physical exercise. Furthermore, the observed pathway-specific effects suggest that regional molecular changes and/or the relative proximity of terminals to the induced electric fields influence the outcomes of LI-rTMS on abnormal circuitry.
In dieser Studie wurden Schlaganfallpatienten untersucht, die einen ausgedehnten Infarkt im Versorgungsgebiet der A.cerebri media erlitten und wegen Bewusstseinstrübung (sog. Maligner Mediainfarkt) auf der Neurologischen Intensivstation des Universitätsklinikums Würzburg im Zeitraum von 1991 bis 2005 behandelt wurde, um herauszufinden, welchen Einfluss eine operative Behandlung auf den kritisch erhöhten Hirnsdrucks zusätzlich zur konservativen Intensivtherapie auf Mortalität sowie langfristige Lebensqualität hatte. Insgesamt konnten die Daten von 292 Patienten ausgewertet werden, wovon 259 konservativ und 33 operativ behandelt worden waren. Es zeigte sich insgesamt, dass eine stillschweigende günstige Selektion für eine Trepanation sprach (jünger, eher keine Aphasie, weniger Komorbiditäten). Die Hemikraniektomie senkte die Mortalität in der Akutphase hochsignifikant (K: 22, 4%, T: 3,0%; p=0,009). Sie hatte erwartungsgemäß auch einen positiven Einfluss auf das Vigilanzniveau: die Quote von wachen Patienten war bei Entlassung der Trepanierten um 66% höher als bei Aufnahme, bei den konservativ Behandelten war sie nur um 33,3 % gestiegen. Das vorherrschende Symptom bei Aufnahme und Entlassung war eine motorische bzw. sensomotorische Hemiparese. Der Anteil der Aphasiker bei den 201 konservativ therapierten Patienten, die den stationären Aufenthalt überlebt haben, ist von bei Aufnahme 56,2% auf bei Entlassung 48,6% gesunken. Bei den 32 trepanierten Patienten ist er dagegen mit 50% gleich geblieben, obwohl 2/3 aller Patienten an der nicht-dominanten Hemisphäre operiert worden waren. Es war und ist auch nicht zu erwarten, dass eine Entlastung von Hirndruck qualitativ die hirninfarktbedingten Symptome beseitigt. Die Nachbefragung der Patienten fand im Schnitt 64,7 Monate nach erlittenem Mediainfarkt statt. Inzwischen waren von den 259 konservativ Behandelten 47,1% verstorben, von den 33 Hemikraniektomierten nur 24,2%. Die poststationäre Mortalität im weiteren Verlauf war anteilsmäßig gering (K: 24%, T: 21,2%). Die Überlebensdauer der Trepanierten war dreimal so lang wie die der nicht operierten (K: 11,6 Monate, T: 34,4 Monate). Diese Unterschiede im Langzeitüberleben sind wahrscheinlicher auf die geringeren Komorbiditäten der Trepanierten zurückzuführen, als auf die stattgehabte Operation an sich. Allerdings ist nicht auszuschließen, dass die durch Trepanation frühzeitiger verbesserte Wachheit sich auch günstig auf lebensverkürzende Folgekomplikationen ausgewirkt haben könnte. In der Nachbefragung zeigte sich, dass bezüglich der erworbenen körperlichen Funktionsdefizite, gemessen am Barthel Index, zwischen den beiden Kollektiven kein signifikanter Unterschied bestand. Die ehemals konservativ behandelten Patienten kamen auf durchschnittlich 75, die trepanierten Patienten auf 60 von 100 Punkten. Im Lebensalltag schlägt sich dieser Unterschied von 15 Punkten relevant nieder, aber insgesamt liegen beide Patientenkollektive im Bereich einer leichten bis nicht vorhandenen Abhängigkeit. Die vergleichbaren Langzeitdaten von Patienten mit Mediainfarkt liegen in einem ähnlichen Bereich. Erstmalig werden hier Langzeitdaten solcher Patienten über die Lebensqualität vorgelegt, gemessen mit dem SF-36. Nachvollziehbar zeigte sich ein deutlicher Unterschied zur Lebensqualität der Durchschnittsbevölkerung, insbesondere im Bereich der körperlichen Belastbarkeit. Für uns unerwartet günstig fielen die Antworten auf der eher psychologischen Ebene aus. Es zeigten sich bei allen Punkten des SF-36 keine signifikanten Unterschiede zwischen dem konservativ behandelten und den hemikraniektomierten Patienten, so dass die Operation als solche keinen eigenständigen Einfluss auf die langfristige Lebensqualität nahm. Zusammengefasst verbesserte die osteoklastische Trepanation des raumfordernden malignen Mediainfarkts die Überlebenschance in der Akutphase signifikant, was mit inzwischen publizierten kontrollierten Studienergebnissen übereinstimmt. Der Langzeitverlauf nach überlebter Akutkrankheit gestaltet sich unabhängig von der Trepanation. Es gibt aufgrund der erworbenen Behinderung eine weiterhin relativ hohe längerfristige Sterblichkeit. Bemerkenswert ist, dass die Selbsteinschätzung der Lebensqualität von Patienten mit einer erheblichen infarktbedingen körperlichen Behinderung psychologisch-emotional nur geringfügig von der Selbstwahrnehmung in der nicht- behinderten Durchschnittsbevölkerung. Dass bedeutet, dass Spekulationen über die zukünftige Lebensqualität keinen Einfluss auf die Operationsindikation nehmen sollten.
Niemann–Pick type C disease (NP-C) presents with heterogeneous neurological and psychiatric symptoms. Adult onset is rare and possibly underdiagnosed due to frequent lack of specific and obvious key symptoms. For both early and adolescent/adult onset, the available data from studies and case reports describe a positive effect of Miglustat (symptom relief or stabilization). However, due to the low frequency of NP-C, experience with this therapy is still limited. We describe two adult-onset cases of NP-C. In both cases, vertical supranuclear gaze palsy was not recognized at symptom onset. Correct diagnosis was delayed from onset of symptoms by more than 10 years. The video demonstrates the broad spectrum of symptoms in later stages of the disease. Compared with published data, the treatment outcome observed in our cases after delayed initiation of Miglustat therapy was disappointing, with continuing disease progression in both cases. Thus, early treatment initiation could be necessary to achieve a good symptomatic effect. Hence, early biochemical testing for NP-C should be considered in patients suffering from atypical neurological/neuropsychological and psychiatric symptoms, even in cases of uncertainty.
Startle disease is a rare disorder associated with mutations in GLRA1 and GLRB, encoding glycine receptor (GlyR) α1 and β subunits, which enable fast synaptic inhibitory transmission in the spinal cord and brainstem. The GlyR β subunit is important for synaptic localization via interactions with gephyrin and contributes to agonist binding and ion channel conductance. Here, we have studied three GLRB missense mutations, Y252S, S321F, and A455P, identified in startle disease patients. For Y252S in M1 a disrupted stacking interaction with surrounding aromatic residues in M3 and M4 is suggested which is accompanied by an increased EC\(_{50}\) value. By contrast, S321F in M3 might stabilize stacking interactions with aromatic residues in M1 and M4. No significant differences in glycine potency or efficacy were observed for S321F. The A455P variant was not predicted to impact on subunit folding but surprisingly displayed increased maximal currents which were not accompanied by enhanced surface expression, suggesting that A455P is a gain-of-function mutation. All three GlyR β variants are trafficked effectively with the α1 subunit through intracellular compartments and inserted into the cellular membrane. In vivo, the GlyR β subunit is transported together with α1 and the scaffolding protein gephyrin to synaptic sites. The interaction of these proteins was studied using eGFP-gephyrin, forming cytosolic aggregates in non-neuronal cells. eGFP-gephyrin and β subunit co-expression resulted in the recruitment of both wild-type and mutant GlyR β subunits to gephyrin aggregates. However, a significantly lower number of GlyR β aggregates was observed for Y252S, while for mutants S321F and A455P, the area and the perimeter of GlyR β subunit aggregates was increased in comparison to wild-type β. Transfection of hippocampal neurons confirmed differences in GlyR-gephyrin clustering with Y252S and A455P, leading to a significant reduction in GlyR β-positive synapses. Although none of the mutations studied is directly located within the gephyrin-binding motif in the GlyR β M3-M4 loop, we suggest that structural changes within the GlyR β subunit result in differences in GlyR β-gephyrin interactions. Hence, we conclude that loss- or gain-of-function, or alterations in synaptic GlyR clustering may underlie disease pathology in startle disease patients carrying GLRB mutations.
Background:
Inhibition of early platelet adhesion by blockade of glycoprotein-IB (GPIb) protects mice from ischemic stroke. To elucidate underlying mechanisms in-vivo, infarct development was followed by ultra-high field MRI at 17.6 Tesla.
Methods:
Cerebral infarction was induced by transient-middle-cerebral-artery-occlusion (tMCAO) for 1 hour in C57/BL6 control mice (N = 10) and mice treated with 100 mg Fab-fragments of the GPIb blocking antibody p0p/B 1 h after tMCAO (N = 10). To control for the effect of reperfusion, additional mice underwent permanent occlusion and received anti-GPIb treatment (N = 6; pMCAO) or remained without treatment (N = 3; pMCAO). MRI 2 h and 24 h after MCAO measured cerebral-blood-flow (CBF) by continuous arterial-spin labelling, the apparent-diffusion-coefficient (ADC), quantitative-T2 and T2-weighted imaging. All images were registered to a standard mouse brain MRI atlas and statistically analysed voxel-wise, and by cortico-subcortical ROI analysis.
Results:
Anti-GPIb treatment led to a relative increase of postischemic CBF vs. controls in the cortical territory of the MCA (2 h: 44.2 +/- 6.9 ml/100g/min versus 24 h: 60.5 +/- 8.4; p = 0.0012, F((1,18)) = 14.63) after tMCAO. Subcortical CBF 2 h after tMCAO was higher in anti-GPIb treated animals (45.3 +/- 5.9 vs. controls: 33.6 +/- 4.3; p = 0.04). In both regions, CBF findings were clearly related to a lower probability of infarction (Cortex/Subcortex of treated group: 35%/65% vs. controls: 95%/100%) and improved quantitative-T2 and ADC. After pMCAO, anti-GPIb treated mice developed similar infarcts preceded by severe irreversible hypoperfusion as controls after tMCAO indicating dependency of stroke protection on reperfusion.
Conclusion:
Blockade of platelet adhesion by anti-GPIb-Fab-fragments results in substantially improved CBF early during reperfusion. This finding was in exact spatial correspondence with the prevention of cerebral infarction and indicates in-vivo an increased patency of the microcirculation. Thus, progression of infarction during early ischemia and reperfusion can be mitigated by anti-platelet treatment.
Transgenic mice bred on C57Bl/6 or Sv/129 genetic background are frequently used in stroke research. It is well established that variations in cerebrovascular anatomy and hemodynamics can influence stroke outcome in different inbred mouse lines. We compared stroke development in C57Bl/6 and Sv/129 mice in the widely used model of transient middle cerebral artery occlusion (tMCAO) by multimodal ultra-high field magnetic resonance imaging (MRI). C57Bl/6 and Sv/129 mice underwent 60 min of tMCAO and were analyzed by MRI 2 h and 24 h afterwards. Structural and functional images were registered to a standard anatomical template. Probability maps of infarction were rendered by automated segmentation from quantitative T2-relaxometric images. Whole-brain segmentation of infarction was accomplished manually on high-resolution T2-weighted (T2-w) RARE images. Cerebral perfusion (cerebral blood flow, CBF) was measured quantitatively by modified continuous arterial-spin-labeling (CASL) and apparent diffusion coefficients (ADC) by spin-echo diffusion-weighted imaging (DWI). Probabilities of cortical (95.1% ± 3.1 vs. 92.1% ± 2.5; p > 0.05) and subcortical (100% vs. 100%; p > 0.05) infarctions at 24 h were similar in both groups as was the whole-brain volumetric extent of cerebral infarction. In addition, CBF and ADC values did not differ between C57Bl/6 and Sv/129 mice at any time point or region of interest. The C57Bl/6 and Sv/129 genetic background is no major confounding factor of infarct size and cerebral perfusion in the tMCAO model.
Background and Objective
This updated systematic review evaluated the efficacy, tolerability and safety of opioids compared to placebo in non‐malignant chronic low back pain.
Databases and Data Treatment
Clinicaltrials.gov, CENTRAL, MEDLINE and PsycINFO were searched from October 2013 to May 2019. Randomized controlled trials comparing opioids with placebo and at least 4 weeks of double‐blinded duration were analysed. Primary outcomes were pain relief of 50% or greater, disability, tolerability and safety. Effects were summarized by a random effects model using risk differences or standardized mean differences. We added nine new studies with 2,980 participants for a total of 21 studies with 7,650 participants. Study duration ranged between 4 and 15 weeks. Studies with a parallel and cross‐over design: Based on very low to low‐quality evidence, opioids provided no clinically relevant pain relief of 50% or greater, but a clinically relevant reduction of disability compared to placebo. Enriched enrolment randomized withdrawal (EERW) design: Based on very low to low‐quality evidence, opioids provided a clinically relevant pain relief of 50% or greater, but not a clinically relevant reduction of disability compared to placebo. There was no clinically relevant harm with regard to serious adverse events by opioids compared to placebo in studies with parallel/cross‐over and EERW design. There was a relevant harm with regard to drop out rates due to adverse events in studies with parallel/cross‐over, but not in studies with EERW design.
Conclusions
Opioids may provide a safe and clinically relevant pain relief for 4–15 weeks in highly selected patients.
Significance
Within the context of randomized controlled trials of 4–15 weeks, opioids provided a clinically relevant pain relief of 30% or greater and a clinically relevant reduction of disability compared to placebo in non‐malignant chronic low back pain. Number needed to treat for an additional drop out due to side effects was 11 (95% confidence interval: 6–33). Assessment of abuse and addiction was incomplete. The frequency of serious adverse events including deaths did not differ from placebo.
LSVT-BIG therapy in Parkinson's disease: physiological evidence for proprioceptive recalibration

Background
There is growing evidence for proprioceptive dysfunction in patients with Parkinson's disease (PD). The Lee Silvermann Voice Treatment-BIG therapy (LSVT-BIG), a special training program aiming at an increase of movement amplitudes in persons with PD (PwPD), has shown to be effective on motor symptoms. LSVT-BIG is conceptionally based on improving bradykinesia, in particular the decrement of repetitive movements, by proprioceptive recalibration.
Objective
To assess proprioceptive impairment in PwPD as compared to matched controls and to probe potential recalibration effects of the LSVT-BIG therapy on proprioception.
Methods
Proprioceptive performance and fine motor skills were assessed in 30 PwPD and 15 matched controls. Measurements with significant impairment in PwPD were chosen as outcome parameters for a standardized 4 weeks amplitude-based training intervention (LSVT-BIG) in 11 PwPD. Proprioceptive performance served as primary outcome measure. Secondary outcome measures included the motor part of the MDS-UPDRS, the nine-hole-peg test, and a questionnaire on quality of life. Post-interventional assessments were conducted at weeks 4 and 8. Results Compared to the control group, PwPD showed significantly larger pointing errors. After 4 weeks of LSVT-BIG therapy and even more so after an additional 4 weeks of continued training, proprioceptive performance improved significantly. In addition, quality of life improved as indicated by a questionnaire.
Conclusion
LSVT-BIG training may achieve a recalibration of proprioceptive processing in PwPD. Our data indicates a probable physiological mechanism of a symptom-specific, amplitude-based behavioral intervention in PwPD.
Beim idiopathischen Parkinson Syndrom (IPS) gewinnen nicht-motorische Symptome in Forschung und Klinik zunehmend an Bedeutung. So findet sich in der Literatur vermehrt Evidenz, dass die Propriozeption bei Patienten mit IPS (PmIPS) gestört ist. Verschiedene klinische und neuroanatomische Studien weisen darauf hin, dass es beim IPS zu einer fehlerhaften sensomotorischen Integration von propriozeptiven Informationen in den Basalganglien kommt. Zudem gibt es Hinweise, dass die passiv-sensible Wahrnehmung von Propriozeption pathologisch verändert ist. Außerdem wird vermutet, dass durch propriozeptives Training eine Verbesserung der Parkinsonsymptomatik erreicht werden kann. Ein spezielles Trainingsprogramm, die LSVT-BIG-Therapie, bei der gezielt trainiert wird, Bewegungen mit einer großen Amplitude durchzuführen, konnte motorische Symptome und Mobilität beim IPS effektiv verbessern.
In der vorliegenden Arbeit stellten wir folgende Hypothesen auf: Das IPS geht mit einer fehlerhaften sensomotorischen Integration von Propriozeption einher. Die afferente propriozeptive Wahrnehmung ist ebenfalls pathologisch verändert. Eine propriozeptive Rekalibrierung ist mithilfe der LSVT-BIG-Therapie möglich.
Für die Überprüfung dieser Hypothesen schlossen wir 30 PmIPS und 15 gesunde Probanden in unsere Fall-Kontroll-Studie ein und führten eine Eingangsuntersuchung durch. 11 PmIPS absolvierten anschließend eine vierwöchige LSVT-BIG-Therapie. Die Folgeuntersuchungen fanden 4 und 8 Wochen nach der Eingangsuntersuchung statt.

Diese beinhalteten neuropsychologische Testungen, außerdem die Bestimmung der Lebensqualität, die Erhebung des motorischen Teils der Movement Disorder Society Unified Parkinson´s Disease Rating Scale (MDS-UPDRS III), Untersuchungen zur Feinmotorik, die Durchführung einer diagnostischen Transkraniellen Magnetstimulation (TMS) sowie Testverfahren zur Propriozeption, darunter sowohl Zeigeversuche, als auch die Bestimmung der Position einer Extremität, ohne visuelle Kontrolle.
Die Ergebnisse zeigten, dass die IPS-Gruppe gegenüber der gesunden Kontrollgruppe signifikant größere Zeigefehler machte, wohingegen die Bestimmung der Position einer Extremität in beiden Gruppen vergleichbar präzise möglich war. Zusätzlich zeigte eine von sieben Messungen der Feinmotorik einen signifikanten Unterschied zwischen PmIPS und Kontrollen. Die Messungen der TMS erbrachten hingegen keine signifikant messbaren Unterschiede zwischen den Gruppen.
In den Folgeuntersuchungen nach therapeutischer Intervention ergaben die Zeigeübungen eine signifikante Verbesserung der BIG-Gruppe im Zeitverlauf. Die Untersuchungen zu Feinmotorik und MDS-UPDRS III ergaben zwar eine tendenzielle Verbesserung durch die LSVT-BIG-Therapie, waren jedoch statistisch nicht signifikant.
Die Lebensqualität der PmIPS in der BIG-Gruppe verbesserte sich signifikant nach Intervention.
Die Ergebnisse sprechen für die Hypothese der fehlerhaften propriozeptiven Integration beim Morbus Parkinson. Dies zeigte sich für aktive sensomotorische Tasks, nicht hingegen in der passiv-sensiblen propriozeptiven Testung.
Auch wenn weitere Studien mit größeren Kohorten benötigt werden, legt unsere Studie nahe, dass die LSVT-BIG-Therapie mit einer propriozeptiven Rekalibrierung einhergeht. Damit erklärt sich möglicherweise der nachhaltige Erfolg der Therapie.
Improving quality of life (QoL) is central to amyotrophic lateral sclerosis (ALS) treatment. This Germany-wide, multicenter cross-sectional study analyses the impact of different symptom-specific treatments and ALS variants on QoL. Health-related QoL (HRQoL) in 325 ALS patients was assessed using the Amyotrophic Lateral Sclerosis Assessment Questionnaire 5 (ALSAQ-5) and EuroQol Five Dimension Five Level Scale (EQ-5D-5L), together with disease severity (captured by the revised ALS Functional Rating Scale (ALSFRS-R)) and the current care and therapies used by our cohort. At inclusion, the mean ALSAQ-5 total score was 56.93 (max. 100, best = 0) with a better QoL associated with a less severe disease status (β = −1.96 per increase of one point in the ALSFRS-R score, p < 0.001). “Limb-onset” ALS (lALS) was associated with a better QoL than “bulbar-onset” ALS (bALS) (mean ALSAQ-5 total score 55.46 versus 60.99, p = 0.040). Moreover, with the ALSFRS-R as a covariate, using a mobility aid (β = −7.60, p = 0.001), being tracheostomized (β = −14.80, p = 0.004) and using non-invasive ventilation (β = −5.71, p = 0.030) were associated with an improved QoL, compared to those at the same disease stage who did not use these aids. In contrast, antidepressant intake (β = 5.95, p = 0.007), and increasing age (β = 0.18, p = 0.023) were predictors of worse QoL. Our results showed that the ALSAQ-5 was better-suited for ALS patients than the EQ-5D-5L. Further, the early and symptom-specific clinical management and supply of assistive devices can significantly improve the individual HRQoL of ALS patients. Appropriate QoL questionnaires are needed to monitor the impact of treatment to provide the best possible and individualized care.
Die Charcot-Marie-Tooth Typ 1 Erkrankungen sind eine genetisch heterogene Gruppe, aktuell nicht kurativ therapierbarer, erblicher Neuropathien des Peripheren Nervensystems. Klinische Manifestationen reichen von Sensibilitäts-störungen, verminderten Muskeleigenreflexen, sowie fortschreitenden Lähmungen, bis hin zu Muskelatrophie und bedeuten für die betroffenen Patienten eine starke Einschränkung der Lebensqualität. Anhand früherer Studien wurde Makrophagen, als Teil des angeborenen Immunsystems, eine entscheidende Rolle in der Pathogenese dreier CMT1-Unterformen zugeschrieben. Abgesehen von den morphologischen Manifestationen der demyelinisierenden CMT1-Erkrankungen, wie simultanes Auftreten von Dedifferenzierung, sowie Hypo-, und Demyelinisierung erkrankter Schwann-Zellen, sind pathologische Veränderungen der Domänengliederung der Ranvier’schen Schnürringe betroffener Nervenfasern ebenfalls von der Aktivierung pathogener Makrophagen abhängig.
Auf der Basis verschiedener veröffentlichter Studien, welche sowohl demyelinisierende Erkrankungen des ZNS, aber auch primär durch axonale Schäden gekennzeichnete Erkrankungen des PNS beinhalten, besteht ein möglicher räumlicher Zusammenhang zwischen Architekturstörungen der RS und aktivierten pathogenen Mikrogliazellen bzw. Makrophagen.
In dieser Studie konnte, anhand morphologischer Analysen von peripherem Nervengewebe, in Wt-Mäusen erstmals eine unerwartete präferentielle Lokalisation von Makrophagen im räumlichen Umfeld von RS beobachtet werden. Hierbei scheint, trotz des Fehlens einer direkten Zell-Zell-Interaktion zwischen Makrophagen und RS, vor allem im Hinblick auf die ebenfalls im räumlichen Umfeld von RS nachweisbare EZM und Fibroblasten, eine funktionelle Relevanz der assoziierten Makrophagen für die Aufrechterhaltung der Domänengliederung bzw. elektrophysiologischen Eigenschaften myelinisierter peripherer Nervenfasern denkbar.
Im Gegensatz dazu wurde trotz der signifikanten Zunahme der Makrophagenanzahlen in den drei untersuchten CMT1-Mausmodellen keine erhöhte räumliche Assoziation mit den RS der mutierten Schwann-Zellen beobachtet. Vielmehr konnten anhand des Vergleiches mit wildtypischen Kontrollmäusen signifikant erniedrigte Assoziationsraten beider Strukturen in den CMT1-Modelltieren festgestellt werden. Folglich scheint die von der Einwanderung und Aktivierung pathogener Makrophagen abhängige Störung der Domänengliederung der RS der mutierten Schwann-Zellen, nicht durch eine direkte Interaktion bzw. räumliche Assoziation von Makrophagen mit RS ausgelöst zu werden.
Previous studies by our group revealed that chronic low grade inflammation implicating phagocytosing macrophages is a highly relevant mechanism in the pathogenesis of Charcot-Marie-Tooth disease. The lack of CSF-1, the primary regulator of macrophage function and survival, led to a robust and persistent amelioration of the phenotype in two authentic mouse models of CMT. Moreover, a close contact between CSF-1 producing fibroblasts and endoneurial macrophages carrying CSF-1R has been confirmed in nerve biopsies of CMT patients, further supporting the clinical significance of this pathway. In the current study we treated 3 distinct mouse models of CMT1: the PMP22tg mice as a model for CMT1A, the P0+/- mice as a model for CMT1B and the Cx32def mice as a model for CMT1X, with a CSF-1R specific kinase (c-FMS) inhibitor (800-1200 mg PLX5622/ kg chow) according to different treatment regimes mimicking an ideal early onset treatment, a late onset treatment and the withdrawal of the drug. Using the above mentioned doses of PLX5622, we documented a dramatic decrease in macrophage numbers in the PNS of all 3 myelin mutants, except for the quadriceps nerve of Cx32def mice. Fibroblast numbers remained unchanged in treated animals. Surprisingly, in spite of the decrease in the number of detrimental macrophages we could not detect an unequivocal phenotypic improvement. CMAP amplitudes were reduced in both wild type and myelin mutant mice treated with CSF-1R inhibitor in comparison to untreated littermates. Corresponding to the electrophysiological findings, the axon number and the percentage of large diameter axons were reduced in the quadriceps nerve of treated P0+/- and Cx32def mice. By contrast we observed a higher number of fully myelinated axons, in parallel with a decrease in the percentage of demyelinated (and hypermyelinated in PMP22tg mice) fibers in the ventral roots of P0+/- mice treated with CSF-1R inhibitor from 3 months up to 6 months of age and PMP22tg animals treated from 9 months up to 15 months of age. Our results indicate that CSF-1R inhibitor has the potential to improve the demyelinating phenotype of at least two models of CMT1. Nevertheless, further studies are necessary (for example with lower doses of the inhibitor) to minimize or even eliminate the putative neurotoxic effect we observed with high dose treatment conditions.
Introduction: Striatal dopamine depletion disrupts basal ganglia function and causes Parkinson’s disease (PD). The pathophysiology of the dopamine-dependent relationship between basal ganglia signaling and motor control, however, is not fully understood. We obtained simultaneous recordings of local field potentials (LFPs) from the subthalamic nucleus (STN) and electromyograms (EMGs) in patients with PD to investigate the impact of dopaminergic state and movement on long-range beta functional connectivity between basal ganglia and lower motor neurons.
Methods: Eight PD patients were investigated 3 months after implantation of a deep brain stimulation (DBS)-system capable of recording LFPs via chronically-implanted leads (Medtronic, ACTIVA PC+S®). We analyzed STN spectral power and its coherence with EMG in the context of two different movement paradigms (tonic wrist extension vs. alternating wrist extension and flexion) and the effect of levodopa (L-Dopa) intake using an unbiased data-driven approach to determine regions of interest (ROI).
Results: Two ROIs capturing prominent coherence within a grand average coherogram were identified. A trend of a dopamine effect was observed for the first ROI (50–150 ms after movement start) with higher STN-EMG coherence in medicated patients. Concerning the second ROI (300–500 ms after movement start), an interaction effect of L-Dopa medication and movement task was observed with higher coherence in the isometric contraction task compared to alternating movements in the medication ON state, a pattern which was reversed in L-Dopa OFF.
Discussion: L-Dopa medication may normalize functional connectivity between remote structures of the motor system with increased upper beta coherence reflecting a physiological restriction of the amount of information conveyed between remote structures. This may be necessary to maintain simple movements like isometric contraction. Our study adds dynamic properties to the complex interplay between STN spectral beta power and the nucleus’ functional connectivity to remote structures of the motor system as a function of movement and dopaminergic state. This may help to identify markers of neuronal activity relevant for more individualized programming of DBS therapy.
In der vorliegenden Studie wurden 32 Patienten (19 Frauen, 13 Männer, medianes Alter 50 Jahren, Spanne: 26-83 Jahre) mit einem klinisch akralen neuropathischen Schmerzsyndrom unterschiedlicher Genese mittels QST, PREP und Hautbiopsie untersucht. Unser Patientenkollektiv bestand aus drei Subgruppen: sechsen Patienten erfüllten die Kriterien einer SFN, acht Patienten hatten eine Neuropathie der großkalibrigen Nervenfasern mit zusätzlicher Beeinträchtigung der kleinkalibrigen Nervenfasern und weitere acht Patienten hatten ein akrales Schmerzsyndrom mit neuropathischen Charakteristika, ohne vorbekannte Diagnose einer Neuropathie der groß- oder kleinkalibrigen Nervenfasern. Die Patienten wurden mittels klinischer neurologischer Untersuchung, elektrophysiologischer Tests, QST, PREP und Hautbiopsie untersucht. Die Patientendaten wurden jeweils mit Daten großer Kontrollgruppen verglichen, die wir in unserer Klinik unter Angehörigen und Freunden unserer Patienten mit deren Einwilligung rekrutiert hatten.
QST und die Hautbiopsie waren bei Patienten mit SFN und PNP jeweils auffällig, bei akralem Schmerzsyndrom unklarer Ätiologie hingegen unauffällig. Nach elektrischer kutaner Stimulation aller drei Körperregionen zeigte sich eine Amplitudenminderung der PREP-Reizantwort in allen Patientensubgruppen (7,5 µV in der SFN-Gruppe, 3,8 µV in der PNP-Gruppe, und 11,3 µV bei den Patienten mit akralem Schmerzsyndrom). Somit konnten wir zeigen, dass eine Kleinfaserpathologie in der Studienpopulation von Patienten mit neuropathischem Schmerzsyndrom besteht. Nur die Amplitudenminderung der PREP bildet diese Pathologie ab.
Diese Daten erlauben uns die eingangs aufgestellte Hypothese, dass PREP zur Diagnostik bei Frage nach Kleinfaserbeteiligung geeignet ist, positiv zu belegen. PREP ist eine nicht-invasive Methode für die Evaluation der Funktion v.a. der Aδ-Faser mit standardisiertem Ablaufprotokoll zur Erhebung von reproduzierbaren Daten. Sie kann bei Patienten mit der Anamnese eines akralen neuropathischen Schmerzsyndroms einen objektiven Hinweis auf eine Dysfunktion der kleinkalibrigen Nervenfasern, auch wenn bereits etablierte Methoden (QST und Hautbiopsie) unauffällig bleiben, erbringen. Entsprechend können die PREP eine wertvolle Ergänzung der klinischen Untersuchungsbatterie für die Evaluation der Funktion der kleinkalibrigen Nervenfasern sein.
Objective: Gait adaptation to environmental challenges is fundamental for independent and safe community ambulation. The possibility of precisely studying gait modulation using standardized protocols of gait analysis closely resembling everyday life scenarios is still an unmet need.
Methods: We have developed a fully-immersive virtual reality (VR) environment where subjects have to adjust their walking pattern to avoid collision with a virtual agent (VA) crossing their gait trajectory. We collected kinematic data of 12 healthy young subjects walking in real world (RW) and in the VR environment, both with (VR/A+) and without (VR/A-) the VA perturbation. The VR environment closely resembled the RW scenario of the gait laboratory. To ensure standardization of the obstacle presentation the starting time speed and trajectory of the VA were defined using the kinematics of the participant as detected online during each walking trial.
Results: We did not observe kinematic differences between walking in RW and VR/A-, suggesting that our VR environment per se might not induce significant changes in the locomotor pattern. When facing the VA all subjects consistently reduced stride length and velocity while increasing stride duration. Trunk inclination and mediolateral trajectory deviation also facilitated avoidance of the obstacle.
Conclusions: This proof-of-concept study shows that our VR/A+ paradigm effectively induced a timely gait modulation in a standardized immersive and realistic scenario. This protocol could be a powerful research tool to study gait modulation and its derangements in relation to aging and clinical conditions.
Gait Initiation in Parkinson’s Disease: Impact of Dopamine Depletion and Initial Stance Condition

Postural instability, in particular at gait initiation (GI), and resulting falls are a major determinant of poor quality of life in subjects with Parkinson’s disease (PD). Still, the contribution of the basal ganglia and dopamine on the feedforward postural control associated with this motor task is poorly known. In addition, the influence of anthropometric measures (AM) and initial stance condition on GI has never been consistently assessed. The biomechanical resultants of anticipatory postural adjustments contributing to GI [imbalance (IMB), unloading (UNL), and stepping phase) were studied in 26 unmedicated subjects with idiopathic PD and in 27 healthy subjects. A subset of 13 patients was analyzed under standardized medication conditions and the striatal dopaminergic innervation was studied in 22 patients using FP-CIT and SPECT. People with PD showed a significant reduction in center of pressure (CoP) displacement and velocity during the IMB phase, reduced first step length and velocity, and decreased velocity and acceleration of the center of mass (CoM) at toe off of the stance foot. All these measurements correlated with the dopaminergic innervation of the putamen and substantially improved with levodopa. These results were not influenced by anthropometric parameters or by the initial stance condition. In contrast, most of the measurements of the UNL phase were influenced by the foot placement and did not correlate with putaminal dopaminergic innervation. Our results suggest a significant role of dopamine and the putamen particularly in the elaboration of the IMB phase of anticipatory postural adjustments and in the execution of the first step. The basal ganglia circuitry may contribute to defining the optimal referent body configuration for a proper initiation of gait and possibly gait adaptation to the environment.
Freezing of gait (FOG) is a sudden episodic inability to produce effective stepping despite the intention to walk. It typically occurs during gait initiation (GI) or modulation and may lead to falls. We studied the anticipatory postural adjustments (imbalance, unloading, and stepping phase) at GI in 23 patients with Parkinson’s disease (PD) and FOG (PDF), 20 patients with PD and no previous history of FOG (PDNF), and 23 healthy controls (HCs). Patients performed the task when off dopaminergic medications. The center of pressure (CoP) displacement and velocity during imbalance showed significant impairment in both PDNF and PDF, more prominent in the latter patients. Several measurements were specifically impaired in PDF patients, especially the CoP displacement along the anteroposterior axis during unloading. The pattern of segmental center of mass (SCoM) movements did not show differences between groups. The standing postural profile preceding GI did not correlate with outcome measurements. We have shown impaired motor programming at GI in Parkinsonian patients. The more prominent deterioration of unloading in PDF patients might suggest impaired processing and integration of somatosensory information subserving GI. The unaltered temporal movement sequencing of SCoM might indicate some compensatory cerebellar mechanisms triggering time-locked models of body mechanics in PD.
Parkinson’s Disease (PD) constitutes a major healthcare burden in Europe. Accounting for aging alone, ~700,000 PD cases are predicted by 2040. This represents an approximately 56% increase in the PD population between 2005 and 2040, with a consequent rise in annual disease‐related medical costs. Gait and balance disorders are a major problem for patients with PD and their caregivers, mainly because to their correlation with falls. Falls occur as a result of a complex interaction of risk factors. Among them, Freezing of Gait (FoG) is a peculiar gait derangement characterized by a sudden and episodic inability to produce effective stepping, causing falls, mobility restrictions, poor quality of life, and increased morbidity and mortality. Between 50–70% of PD patients have FoG and/or falls after a disease duration of 10 years, only partially and inconsistently improved by dopaminergic treatment and Deep Brain Stimulation (DBS). Treatment-induced worsening has been also observed under certain conditions. Effective treatments for gait disturbances in PD are lacking, probably because of the still poor understanding of the supraspinal locomotor network.
In my thesis, I wanted to expand our knowledge of the supraspinal locomotor network and in particular the contribution of the basal ganglia to the control of locomotion. I believe this is a key step towards new preventive and personalized therapies for postural and gait problems in patients with PD and related disorders. In addition to patients with PD, my studies also included people affected by Progressive Supranuclear Palsy (PSP). PSP is a rare primary progressive parkinsonism characterized at a very early disease stage by poor balance control and frequent backwards falls, thus providing an in vivo model of dysfunctional locomotor control.
I focused my attention on one of the most common motor transitions in daily living, the initiation of gait (GI). GI is an interesting motor task and a relevant paradigm to address balance and gait impairments in patients with movement disorders, as it is associated with FoG and high risk of falls. It combines a preparatory (i.e., the Anticipatory Postural Adjustments [APA]) and execution phase (the stepping) and allows the study of movement scaling and timing as an expression of muscular synergies, which follow precise and online feedback information processing and integration into established feedforward patterns of motor control.
By applying a multimodal approach that combines biomechanical assessments and neuroimaging investigations, my work unveiled the fundamental contribution of striatal dopamine to GI in patients with PD. Results in patients with PSP further supported the fundamental role of the striatum in GI execution, revealing correlations between the metabolic intake of the left caudate nucleus with diverse GI measurements. This study also unveiled the interplay of additional brain areas in the motor control of GI, namely the Thalamus, the Supplementary Motor Area (SMA), and the Cingulate cortex. Involvement of cortical areas was also suggested by the analysis of GI in patients with PD and FoG. Indeed, I found major alterations in the preparatory phase of GI in these patients, possibly resulting from FoG-related deficits of the SMA. Alterations of the weight shifting preceding the stepping phase were also particularly important in PD patients with FoG, thus suggesting specific difficulties in the integration of somatosensory information at a cortical level. Of note, all patients with PD showed preserved movement timing of GI, possibly suggesting preserved and compensatory activity of the cerebellum. Postural abnormalities (i.e., increased trunk and thigh flexion) showed no relationship with GI, ruling out an adaptation of the motor pattern to the altered postural condition. In a group of PD patients implanted with DBS, I further explored the pathophysiological functioning of the locomotor network by analysing the timely activity of the Subthalamic Nucleus (STN) during static and dynamic balance control (i.e., standing and walking). For this study, I used novel DBS devices capable of delivering stimulation and simultaneously recording Local Field Potentials (LFP) of the implanted nucleus months and years after surgery. I showed a gait-related frequency shift in the STN activity of PD patients, possibly conveying cortical (feedforward) and cerebellar (feedback) information to mesencephalic locomotor areas. Based on this result, I identified for each patient a Maximally Informative Frequency (MIF) whose power changes can reliably classify standing and walking conditions. The MIF is a promising input signal for new DBS devices that can monitor LFP power modulations to timely adjust the stimulation delivery based on the ongoing motor task (e.g., gait) performed by the patient (adaptive DBS).
Altogether my achievements allowed to define the role of different cortical and subcortical brain areas in locomotor control, paving the way for a better understanding of the pathophysiological dynamics of the supraspinal locomotor network and the development of tailored therapies for gait disturbances and falls prevention in PD and related disorders.
In der Behandlung neuropathischer und anderer chronischer Schmerzen werden trizyklische Antidepressiva bereits mit Erfolg eingesetzt, die nebenwirkungsärmeren SSRI zeigen jedoch nur einen mäßigen Erfolg. In dieser Studie gingen wir der Frage nach, inwieweit 5-HTT -/- Mäuse, die als Modell einer lebenslangen Behandlung mit SSRI gelten, in einem chronisch entzündlichen Schmerzmodell ein anderes Schmerzverhalten zeigen als Wild-typen und ob sich auf neuronaler Ebene durch das Ausschalten des 5-HT Transporters Ursachen für ein geändertes Schmerzverhalten finden lassen. Von besonderem Interesse war dabei auch, welche Rolle 5-HT in der peripheren Schmerzvermittlung zukommt. Mit standardisierten Testverfahren wurden die 5-HTT -/- Mäuse und Wildtypmäuse nach i.pl. Injektion von CFA auf zwei Schmerzqualitäten hin untersucht. Die Schmerzschwelle für taktile Reize wurde mit von Frey Haaren bestimmt, zur Testung der Hitzehyperalgesie wurde eine Infrarotwärmequelle benutzt. Anschließend wurden an dem Gewebe immunhistochemische Analysen durchgeführt und mittels HPLC der Gehalt an 5-HT in verschiedenen Gewebeproben bestimmt. Es konnte gezeigt werden, dass Mäuse mit dem Genotyp 5-HTT -/- gegenüber dem Wildtyp von einer durch CFA-Injektion induzierten Hitzehyperalgesie weitgehend unbeeinträchtigt bleiben. Gleichzeitig bestand bei den KO-Mäusen im Vergleich zu den Wildtypen eine deutlichere Abnahme der Hautinnervation sowie eine stärker ausgeprägte Verletzung von DRG-Neuronen, entsprechend einer erhöhten neuronalen Vulnerabilität gegenüber CFA. Im Gewebe der KO-Mäuse fand sich durchweg weniger 5-HT als bei Wildtypen, in DRG-Neuronen der KO-Mäuse war weiterhin weniger BDNF detektierbar. Wir postulieren, dass für die reduzierte Hitzehyperalgesie bei den KO-Mäusen unter anderem der geringere Gewebespiegel von 5-HT und damit folglich in einer Art Ursachen-Wirkungskette auch die geringeren Gewebespiegel von BDNF und 5-HIAA mit ihren entsprechenden Auswirkungen verantwortlich sind. 5-HTT -/- Mäuse als Modell für eine lebenslange Behandlung mit SSRI sind also nicht nur wie kürzlich beschrieben im neuropathischen Schmerzmodell, sondern auch in einem chronisch entzündlichen Schmerzmodell weitgehend vor einer Hitzehyperalgesie geschützt. Unter der Berücksichtigung dieser Daten sollte daher der Einsatz von SSRI in der Behandlung chronischer Schmerzen erneut überprüft werden.
Die Charcot-Marie-Tooth-Neuropathie umfasst eine heterogene Gruppe von erblichen unter anderem demyelinisierenden Erkrankungen des peripheren Nervensystems. Trotz ihrer hohen Prävalenz von 1:2.500 gibt es bis dato keine kausalen Therapiemöglichkeiten. Durch den progressiven Krankheitsverlauf wird die Lebensqualität der Patienten stetig gemindert; der fortschreitende Verlust der Muskelkraft und Störungen des Gangbildes sind besonders belastend.
Ursächlich für die CMT1-Neuropathie sind unter anderem Mutationen in Genen, die für Moleküle des Myelins von Schwannzellen codieren. Diese Mutationen führen zu einer verminderten Stabilität und Funktion des Myelins und so letzten Endes zu einer Demyelinisierung und axonalen Schädigung der peripheren Nerven. Weitere Studien in CMT1-Mausmodellen zeigten jedoch, dass nicht nur die verringerte Myelinstabilität sondern auch eine durch das Immunsystem vermittelte geringgradige Entzündungsreaktion für die Symptome ursächlich sein könnte. Hier spielen vor allem Makrophagen eine zentrale Rolle. Das Zytokin CSF-1 aktiviert die Makrophagen und verursacht so eine Demyelinisierung der peripheren Nerven. In P0het und Cx32def Mausmodellen konnte nachgewiesen werden, dass eine medikamentöse Inhibition des CSF-1-Rezeptors an Makrophagen zu einem verbesserten Nervphänotypen und einer deutlichen Abmilderung des Krankheitsbildes führte.
In dieser Arbeit wurden in P0het und Cx32def Mausmodellen weiterführende Behandlungsstudien mit einem CSF-1-RI durchgeführt, die untersuchen, zu welchem Zeitpunkt innerhalb des Krankheitsverlaufs (therapeutisch oder präventiv) eine erfolgreiche Therapie noch möglich ist und ob bei einem früheren Beginn eine noch bessere Wirkung erzielt werden kann.
Abhängig von den verschiedenen Start- und Endpunkten waren unterschiedliche Ergebnisse zu beobachten: Hinsichtlich der klinischen Parameter wie der Greifkraft und der Anzahl an abnormal innervierten Synapsen zeigten die Tiere im präventiven Behandlungszweig in beiden Mausmodellen das beste Ergebnis im Vergleich zu den Kontrolltieren. Diese substantielle Verbesserung ließ sich unabhängig von einem Makrophagen-Reflux sogar noch 6 Monate nach Behandlungsabbruch nachweisen.
Bezüglich der endoneuralen Makrophagendepletion war sowohl in den P0het als auch den Cx32def Tieren im präventiven sowie im therapeutischen Behandlungszweig eine signifikante Verbesserung zu beobachten.
Diese Ergebnisse heben ein weiteres Mal die Bedeutung der Makrophagen als Teil einer Entzündungsreaktion in der Pathogenese der CMT1-Neuropathie hervor. Des Weiteren konnte die These gefestigt werden, dass eine Inhibition des CSF-1-Rezeptors zu verbesserten histopathologischen sowie funktionellen Parametern führt. Um ein gutes Ansprechen auf die Therapie zu erzielen, müssen ein möglichst früher Therapiebeginn sowie eine nachhaltige Behandlungsdauer gewährleistet sein.
Das koinhibitorische Molekül B7-H1 beeinflusst adaptive Immunantworten und ist vermutlich an den Mechanismen zur Aufrechterhaltung peripherer Toleranz und der Limitierung inflammatorischen Schadens beteiligt. Zusätzlich kommt DZ eine entscheidende Bedeutung in der Entwicklung, Aufrechterhaltung und Regulation ZNS-spezifischer Autoimmunität und Inflammationsprozessen zu. Um den B7-H1/PD-1-Signalweg eingehender zu untersuchen, wurden adaptive Immunantworten und die Zielorgan-spezifische Infiltration im Modell der MOG35-55-induzierten EAE analysiert, einem Tiermodell der MS, das durch neurologische Schädigungen und progressive Paralyse bedingt durch die inflammatorische Demyelinisierung im ZNS charakterisiert ist. Im Vergleich zu Wildtyptieren zeigten B7-H1-/- Mäuse einen beschleunigten Krankheitsbeginn und eine signifikante Steigerung des Schweregrads der EAE. Periphere MOG35-55-spezifische IFNg-/IL-17-Immunzellantworten traten in B7-H1-/- Mäusen verfrüht und verstärkt auf, klangen allerdings auch schneller ab. Im ZNS persistierte jedoch eine signifikant höhere Anzahl aktivierter, Neuroantigen-spezifischer T-Zellen während allen Phasen der EAE, wobei diese Zellen ebenfalls größere Mengen proinflammatorischer Zytokine sezernieren konnten. Experimente mit APZ-assoziiertem B7-H1, die einen direkten inhibitorischen Effekt auf die Aktivierung und Proliferation MOG35-55-spezifischer Effektorzellen zeigten, unterstützen die Hypothese, dass parenchymale Expression von B7-H1 ausschlaggebend für das Schicksal von T-Zellen im Zielorgan ist. B7-H1 stellt damit ein Schlüsselmolekül für die Kontrolle parenchymaler Immunreaktionen dar. Nachdem die Relevanz von B7-H1 auf APZ in vitro bewiesen werden konnte, wurde der Einfluss von B7-H1 auf systemisch oder intrazerebral injizierten DZ mit immunogenem oder tolerogenem Phänotyp untersucht. Intravenöse Applikation von tolerogenen B7-H1-/- DZ resultierte in einer besseren Protektion gegen EAE, und dieser Effekt war von einer gesteigerten Produktion Tr1-/Th2-typischer Zytokine sowie einer verstärkten Sekretion von IL-4 und IL-13 durch CD1d-restringierte T-Zellen in der Peripherie begleitet. Die Anzahl Neuroantigen-spezifischer T-Zellen, die proinflammatorische Zytokine sezernierten, war dementsprechend sowohl in der Peripherie als auch im ZNS reduziert. In diesem Zusammenhang konnte für B7-H1 eine wesentliche Beteiligung an der Inhibition der Aktivierung antigen-spezifischer, regulatorischer T-Zellen und CD1d-restringierter T-Zellen gefunden werden. Bei der Injektion intrazerebraler DZ bewirkten tolerogene DZ im Vergleich zu immunogenen DZ eine Reduktion der ZNS-Infiltration mit CD4+ T-Zellen in der frühen Phase der Erkrankung. Außerdem konnte eine Veränderung des intrazerebralen Zytokinmilieus von IFNg/IL-17 exprimierenden enzephalitogenen T-Zellen zu IL-10+ regulatorischen T-Zellen gezeigt werden. B7-H1-Defizienz auf APZ verstärkte diesen Effekt und führte dadurch in den Mäusen zur partiellen Protektion gegen klinische Symptome der EAE. Zusätzlich wurde die Beteiligung von B7-H1 an der Rekrutierung und ZNS-lokalisierten Induktion der Proliferation CD8+ regulatorischer T-Zellen durch DZ beschrieben. Unabhängig vom Phänotyp der DZ wurde eine bereits in der frühen Phase vorhandene und dauerhaft expandierende Population von CD8+ T-Zellen im ZNS DZ[B7-H1-/-]-injizierter Mäuse gefunden. Diese Zellen konnten in vitro die Proliferation MOG35-55-spezifischer CD4+ T-Zellen supprimieren und wirkten so mutmaßlich an der Abmilderung der EAE mit. Zusammengefasst zeigen die Ergebnisse dieser Arbeit die entscheidende Bedeutung von B7 H1 auf DZ als immuninhibitorisches Molekül, das sowohl enzephalitogene als auch regulatorische T-Zell-Antworten moduliert und damit zur Limitation von Immunantworten beiträgt.
Die vorliegende Arbeit ist die erste, die sich mit der Frage beschäftigt, mit welcher zur Deeskalation eingesetzten Therapie nach Beendigung einer Eskalationstherapie mit Mitoxantron am besten Krankheitsstabilität erreicht werden kann bzw. ob Patienten-/Krankheitscharakteristika existieren, die eine bestimmte Nachfolge-Therapie favorisieren.
Trotz neuer Behandlungsmöglichkeiten der hochaktiven MS mit Fingolimod, Natalizumab und Alemtuzumab hat Mitoxantron im klinischen Alltag nach wie vor einen hohen Stellenwert, so dass die Fragestellung dieser Studie weiter relevant ist.
Es zeigten sich keine Patientencharakteristika, die auf eine erfolgsversprechende Therapie in der Deeskalationsphase nach Mitoxantron schließen ließen.
Bei Patienten, bei denen während der Eskalation mit Mitoxantron die Dosis reduziert werden konnte, wurden während der Deeskalationstherapie ein stabilerer Verlauf und weniger Therapiewechsel beobachtet. Bei Patienten, die wegen einer rein chronischen Krankheitsprogredienz eskaliert wurden, trat eine Verschlechterung nach Deeskalation häufiger auf als bei denjenigen, welche wegen Schubaktivität eskaliert wurden.
Die Aussagekraft der Daten wird durch die nur niedrige Anzahl der in diese Studie eingeschlossenen Patienten limitiert. Rekrutierungsprobleme stellten die Hauptursache für die geringe Anzahl der Studienteilnehmer dar.
Topological differences and confounders of mental rotation in cervical dystonia and blepharospasm

Mental rotation (mR) bases on imagination of actual movements. It remains unclear whether there is a specific pattern of mR impairment in focal dystonia. We aimed to investigate mR in patients with cervical dystonia (CD) and blepharospasm (BS) and to assess potential confounders. 23 CD patients and 23 healthy controls (HC) as well as 21 BS and 19 hemifacial spasm (HS) patients were matched for sex, age, and education level. Handedness, finger dexterity, general reaction time, and cognitive status were assessed. Disease severity was evaluated by clinical scales. During mR, photographs of body parts (head, hand, or foot) and a non-corporal object (car) were displayed at different angles rotated within their plane. Subjects were asked to judge laterality of the presented image by keystroke. Both speed and correctness were evaluated. Compared to HC, CD and HS patients performed worse in mR of hands, whereas BS group showed comparable performance. There was a significant association of prolonged mR reaction time (RT) with reduced MoCA scores and with increased RT in an unspecific reaction speed task. After exclusion of cognitively impaired patients, increased RT in the mR of hands was confined to CD group, but not HS. While the question of whether specific patterns of mR impairment reliably define a dystonic endophenotype remains elusive, our findings point to mR as a useful tool, when used carefully with control measures and tasks, which may be capable of identifying specific deficits that distinguish between subtypes of dystonia.
Background
Increasing attention is payed to the contribution of somatosensory processing in motor control. In particular, temporal somatosensory discrimination has been found to be altered differentially in common movement disorders. To date, there have only been speculations as to how impaired temporal discrimination and clinical motor signs may relate to each other. Prior to disentangling this relationship, potential confounders of temporal discrimination, in particular age and peripheral nerve conduction, should be assessed, and a quantifiable measure of proprioceptive performance should be established.
ObjectiveTo assess the influence of age and polyneuropathy (PNP) on somatosensory temporal discrimination threshold (STDT), temporal discrimination movement threshold (TDMT), and behavioral measures of proprioception of upper and lower limbs.
Methods
STDT and TDMT were assessed in 79 subjects (54 healthy, 25 with PNP; age 30–79 years). STDT was tested with surface electrodes over the thenar or dorsal foot region. TDMT was probed with needle electrodes in flexor carpi radialis (FCR) and tibialis anterior (TA) muscle. Goniometer-based devices were used to assess limb proprioception during (i) active pointing to LED markers, (ii) active movements in response to variable visual cues, and (iii) estimation of limb position following passive movements. Pointing (or estimation) error was taken as a measure of proprioceptive performance.
Results
In healthy subjects, higher age was associated with higher STDT and TDMT at upper and lower extremities, while age did not correlate with proprioceptive performance. Patients with PNP showed higher STDT and TDMT values and decreased proprioceptive performance in active pointing tasks compared to matched healthy subjects. As an additional finding, there was a significant correlation between performance in active pointing tasks and temporal discrimination thresholds.
Conclusion
Given their notable impact on measures of temporal discrimination, age and peripheral nerve conduction need to be accounted for if STDT and TDMT are applied in patients with movement disorders. As a side observation, the correlation between measures of proprioception and temporal discrimination may prompt further studies on the presumptive link between these two domains.
(1) Background: The first-line treatment for patients with focal or segmental dystonia with a craniocervical distribution is still the intramuscular injection of botulinum neurotoxin (BoNT). However, some patients experience primary or secondary treatment failure from this potential immunogenic therapy. Deep brain stimulation (DBS) may then be used as a backup strategy in this situation. (2) Methods: Here, we reviewed the current study literature to answer a specific question regarding the efficacy and safety of the use of DBS, particularly for cervical dystonia (CD) and Meige syndrome (MS) in patients with documented treatment failure under BoNT. (3) Results: There are only two studies with the highest level of evidence in this area. Despite this clear limitation, in the context of the narrowly defined research question of this paper, it is possible to report 161 patients with CD or MS who were included in studies that were able to show a statistically significant reduction in dystonic symptoms using DBS. Safety and tolerability data appeared adequate. However, much of the information is based on retrospective observations. (4) Conclusions: The evidence base in this area is in need of further scientific investigation. Most importantly, more randomized, controlled and double-blind trials are needed, possibly including a head-to-head comparison of DBS and BoNT.
Background: Cervical dystonia is a movement disorder causing abnormal postures and movements of the head. While the exact pathophysiology of cervical dystonia has not yet been fully elucidated, a growing body of evidence points to the cerebellum as an important node.
Methods: Here, we examined the impact of cerebellar interference by transcranial magnetic stimulation on finger-tapping related brain activation and neurophysiological measures of cortical excitability and inhibition in cervical dystonia and controls. Bilateral continuous theta-burst stimulation was used to modulate cerebellar cortical excitability in 16 patients and matched healthy controls. In a functional magnetic resonance imaging arm, data were acquired during simple finger tapping before and after cerebellar stimulation. In a neurophysiological arm, assessment comprised motor-evoked potentials amplitude and cortical silent period duration. Theta-burst stimulation over the dorsal premotor cortex and sham stimulation (neurophysiological arm only) served as control conditions.
Results: At baseline, finger tapping was associated with increased activation in the ipsilateral cerebellum in patients compared to controls. Following cerebellar theta-burst stimulation, this pattern was even more pronounced, along with an additional movement-related activation in the contralateral somatosensory region and angular gyrus. Baseline motor-evoked potential amplitudes were higher and cortical silent period duration shorter in patients compared to controls. After cerebellar theta-burst stimulation, cortical silent period duration increased significantly in dystonia patients.
Conclusion: We conclude that in cervical dystonia, finger movements—though clinically non-dystonic—are associated with increased activation of the lateral cerebellum, possibly pointing to general motor disorganization, which remains subclinical in most body regions. Enhancement of this activation together with an increase of silent period duration by cerebellar continuous theta-burst stimulation may indicate predominant disinhibitory effects on Purkinje cells, eventually resulting in an inhibition of cerebello-thalamocortical circuits.
Motor aspects of Parkinson’s disease, such as fluctuations and dyskinesia, can be reliably evaluated using a variety of “wearable” technologies, but practical guidance on objective measurement (OM) and the optimum use of these devices is lacking. Therefore, as a first step, a panel of movement disorder specialists met to provide guidance on how OM could be assessed and incorporated into clinical guidelines. A key aspect of the incorporation of OM into the management of Parkinson’s disease (PD) is defining cutoff values that separate “controlled” from “uncontrolled” symptoms that can be modified by therapy and that relate to an outcome that is relevant to the person with PD (such as quality of life). Defining cutoffs by consensus, which can be subsequently tested and refined, is the first step to optimizing OM in the management of PD. OM should be used by all clinicians that treat people with PD but the least experienced may find the most value, but this requires guidance from experts to allow non-experts to apply guidelines. While evidence is gained for devices that produce OM, expert opinion is needed to supplement the evidence base.
Organ manifestations and long-term outcome of Fabry disease in patients with the GLA haplotype D313Y

Objectives: The severity of Fabry disease is dependent on the type of mutation in the α-galactosidase A (AgalA) encoding gene (GLA). This study focused on the impact of the GLA haplotype D313Y on long-term organ involvement and function.
Setting and participants: In this monocentric study, all participants presenting with the D313Y haplotype between 2001 and 2015 were comprehensively clinically investigated at baseline and during a 4-year follow-up if available. Five females and one male were included.
Primary and secondary outcome measures: Cardiac, nephrological, neurological, laboratory and quality of life data.
Results: AgalA enzyme activity in leucocytes (0.3±0.9 nmol/min/mg protein (mean±SD)) and serum lyso-Gb3 (0.6±0.3 ng/mL at baseline) were in normal range in all patients. Cardiac morphology and function were normal (left-ventricular (LV) ejection fraction 66±8%; interventricular septum 7.7±1.4 mm; LV posterior wall 7.5±1.4 mm; normalised LV mass in MRI 52±9 g/m2; LV global longitudinal strain −21.6±1.9%) and there were no signs of myocardial fibrosis in cardiac MRI. Cardiospecific biomarkers were also in normal range. Renal function was not impaired (estimated glomerular filtration rate MDRD 103±15 mL/min; serum-creatinine 0.75±0.07 mg/dL; cystatin-c 0.71±0.12 mg/L). One female patient (also carrying a Factor V Leiden mutation) had a transitory ischaemic attack. One patient showed white matter lesions in brain MRI, but none had Fabry-associated pain attacks, pain crises, evoked pain or permanent pain. Health-related quality of life analysis revealed a reduction in individual well-being. At long-term follow-up after 4 years, no significant change was seen in any parameter.
Conclusions: The results of the current study suggest that the D313Y genotype does not lead to severe organ manifestations as seen in genotypes known to be causal for classical FD."
Das ON-Freezing ist ein seltenes, aber generell extrem schwer zu therapierendes Phänomen. Es betrifft Parkinson-Patienten mit und ohne THS.
Die derzeitige Literaturlage spiegelt wider, dass es unterschiedliche Strategien gibt, diesem Phänomen zu begegnen. Ein allgemeingültiges Therapiekonzept existiert dabei nicht. Für einige Patienten mit STN-THS konnte durch eine Reduktion der Stimulationsfrequenz eine Besserung der Gangstörung erzielt werden. Andere profitierten vom Einsatz sogenannter Interleaving-Protokolle mit gleichzeitiger Stimulation der Substantia nigra (Sn).
Im Vergleich zu anderen Arbeiten, die keine vorhersagbaren Parameter gefunden oder sich auf Symptome, Ausprägung der Subtypen und Erkrankungsdauer oder
den Zeitpunkt der Erkrankung konzentriert haben, verfolgten wir die Absicht, die Effekte der LF-Stim des STN auf Parkinson-Patienten mit Gangstörung und Freezing-Phänomen zu untersuchen und herauszufinden, ob man Gangparameter identifizieren kann, an Hand derer man das Ansprechen auf eine LF-Stim vorhersagen kann.
Unter der Einschränkung, dass die Zahl der Probanden unserer Studie sehr gering ist, haben wir herausgefunden, dass diejenigen Patienten besser auf eine LF-Stim ansprechen, die unter der Standard-HF-Stim eine signifikant höhere Ganggeschwindigkeit und eine größere Schrittlänge aufzeigen und nur ein intermittierendes Freezing haben.
Darüber hinaus zeigte sich ein besseres Ansprechen der LF-Stim bei Parkinson-Patienten mit akinetisch-rigidem Parkinson-Phänotyp.
Unsere Ergebnisse bestätigen die Annahme, dass sich L-Dopa additiv zur Stimulationstherapie bei manchen Parkinson-Patienten zusätzlich positiv auf die motorischen PD-Symptome auswirken kann. In Bezug auf die Verbesserung der Gangparameter zeigte sich in unseren Ergebnissen allerdings, dass L-Dopa eher eine untergeordnete Rolle spielt.
Aufgrund der niedrigen Anzahl von Respondern in unserer Studie lässt sich daher sicherlich noch keine allgemeingültige Regel ableiten. Es bedarf letztlich weiterer Studien mit größeren Untersuchungszahlen, um unsere Thesen zu stützen und abzusichern.
In jedem Fall wird aber das ON-Freezing auch weiterhin eine therapeutische Herausforderung bleiben.
Natalizumab ist ein humanisierter monoklonaler Antikörper gegen das Oberflächenadhäsionsmolekül Integrin α-4, der zur Therapie von schweren Verlaufsformen der Multiplen Sklerose (MS) zugelassen ist. Integrin α-4/β-1 wird durch Leukozyten exprimiert und steuert deren Extravasation über die Bindung an VCAM-1 (vascular cell adhesion molecule-1) auf Endothelzellen. Natalizumab wirkt über eine Blockade der Leukozytenadhäsion. In einigen Publikationen konnte darüber hinaus gezeigt werden, dass Integrin α-4 auch auf zerebralen Endothelzellen von Mäusen und Ratten exprimiert wird. In der vorliegenden Arbeit wurde die Expression und Funktion von Integrin α-4 in kultivierten primären humanen zerebralen Endothelzellen untersucht.
Die im Rahmen dieser Arbeit an verschiedenen Einzelspenderpräparationen durchgeführten FACS-Analysen zeigten, dass Integrin α-4 in unterschiedlicher Ausprägung auf primären zerebralen Endothelzellisolationen nachzuweisen war. Mit Hilfe immunzytochemischer Färbungen konnte ein spezifisches Verteilungsmuster des Integrin α-4 in Form eines feinen, granulären Musters im Bereich des Zellleibes dokumentiert werden.
In Adhäsionsversuchen zeigten Integrin α-4-exprimierende Endothelzellen nach Zugabe von Natalizumab in niedriger Konzentration eine verminderte Fähigkeit zur Haftung an Fibronectin, einem Bindungspartner in der extrazellulären Matrix. In hohen Konzentrationen dominierte im eingesetzten experimentellen System ein unspezifischer Blockadeeffekt, der auch mit Kontrollantikörpern zu beobachten war.
In MS-Läsionen findet sich auch die lösliche Form des VCAM-1 (sVCAM-1), die möglicherweise mit endothelialem Integrin α-4 interagiert. Daher wurde mit Hilfe von Western-Blot-Untersuchungen die intrazelluläre Signaltransduktion unter Stimulation mit sVCAM-1 untersucht. Es zeigte sich wie in anderen Endothelarten vorbeschrieben eine Aktivierung des p38-MAP-Kinase-Signalweges.
Zusammenfassend wurde demonstriert, dass primäre humane zerebrale Endothelzellen Integrin α-4 exprimieren und dass dieses wahrscheinlich nicht nur für die mechanische Verankerung in der Extrazellulärmatrix eine Bedeutung besitzt, sondern auch als Induktor intrazellulärer Signaltransduktion fungiert, welche die Schrankeneigenschaften zerebraler Endothelzellen beeinflussen könnte.
Myelin formation during peripheral nervous system (PNS) development, and reformation after injury and in disease, requires multiple intrinsic and extrinsic signals. Akt/mTOR signaling has emerged as a major player involved, but the molecular mechanisms and downstream effectors are virtually unknown. Here, we have used Schwann-cell-specific conditional gene ablation of raptor and rictor, which encode essential components of the mTOR complexes 1 (mTORC1) and 2 (mTORC2), respectively, to demonstrate that mTORC1 controls PNS myelination during development. In this process, mTORC1 regulates lipid biosynthesis via sterol regulatory element-binding proteins (SREBPs). This course of action is mediated by the nuclear receptor RXRg, which transcriptionally regulates SREBP1c downstream of mTORC1. Absence of mTORC1 causes delayed myelination initiation as well as hypomyelination, together with abnormal lipid composition and decreased nerve conduction velocity. Thus, we have identified the mTORC1-RXR gamma-SREBP axis controlling lipid biosynthesis as a major contributor to proper peripheral nerve function.
The ganglioside-induced differentiation-associated protein 1 (GDAP1) is a mitochondrial fission factor and mutations in GDAP1 cause Charcot–Marie–Tooth disease. We found that Gdap1 knockout mice (\(Gdap1^{−/−}\)), mimicking genetic alterations of patients suffering from severe forms of Charcot–Marie–Tooth disease, develop an age-related, hypomyelinating peripheral neuropathy. Ablation of Gdap1 expression in Schwann cells recapitulates this phenotype. Additionally, intra-axonal mitochondria of peripheral neurons are larger in \(Gdap1^{−/−}\) mice and mitochondrial transport is impaired in cultured sensory neurons of \(Gdap1^{−/−}\) mice compared with controls. These changes in mitochondrial morphology and dynamics also influence mitochondrial biogenesis. We demonstrate that mitochondrial DNA biogenesis and content is increased in the peripheral nervous system but not in the central nervous system of \(Gdap1^{−/−}\) mice compared with control littermates. In search for a molecular mechanism we turned to the paralogue of GDAP1, GDAP1L1, which is mainly expressed in the unaffected central nervous system. GDAP1L1 responds to elevated levels of oxidized glutathione by translocating from the cytosol to mitochondria, where it inserts into the mitochondrial outer membrane. This translocation is necessary to substitute for loss of GDAP1 expression. Accordingly, more GDAP1L1 was associated with mitochondria in the spinal cord of aged \(Gdap1^{−/−}\) mice compared with controls. Our findings demonstrate that Charcot–Marie–Tooth disease caused by mutations in GDAP1 leads to mild, persistent oxidative stress in the peripheral nervous system, which can be compensated by GDAP1L1 in the unaffected central nervous system. We conclude that members of the GDAP1 family are responsive and protective against stress associated with increased levels of oxidized glutathione.
In einer Vielzahl von epidemiologischen Studien zeigten Patienten, die an einem idiopathischen Parkinson-Syndrom (IPS) erkrankt waren, erniedrigte Vitamin D-Serumspiegel (25-(OH)-Vit D). Die Rolle von Vitamin D im Knochenstoffwechsel ist weitgehend bekannt, allerdings konnten Assoziationen zwischen Vitamin D und chronischen Erkrankungen, die das Nervensystem sowie das kardiovaskuläre und immunologische System betreffen, nachgewiesen werden. In Tiermodellen konnten anti-oxidative Effekte von Vitamin D im Nervensystem gezeigt werden. In den letzten Jahren häuften sich allerdings Studien, die gegen einen direkten Zusammenhang zwischen IPS und Vitamin D sprechen. Demnach stellt sich die Frage, ob dem gehäuften Auftreten eines Vitamin D-Mangels bei IPS-Patienten eine krankheitsspezifische Ursache zugrunde liegt oder ob diese lediglich ein unspezifisches krankheitsbegleitendes Phänomen darstellt.
In der vorliegenden Arbeit wurden in einer retrospektiven Analyse Parkinson-Patienten aus der neurogerontopsychiatrischen Tagesklinik sowie der neurogeriatrischen Frührehastation der Neurologischen Klinik der Universitätsklinik Würzburg hinsichtlich ihres 25-(OH)-Vit D-Serumspiegel mit zwei Kontrollgruppen bestehend aus Patienten mit psychiatrischer bzw. anderweitig neurologischer Erkrankung, die keiner Parkinson-Erkrankung entsprach, verglichen. Im Anschluss wurde auf mögliche Konfounder sowie der Zusammenhang zwischen IPS-Risiko bzw. Krankheitsschwere und 25-(OH)-Vit D-Serumspiegel untersucht.
Der mittlere 25-(OH)-Vit D-Serumspiegel der Neurologie-Gruppe war im Vergleich zur Psychiatrie-Gruppe signifikant niedriger. Der Unterschied zwischen IPS-Gruppe und Psychiatrie- bzw. Neurologie-Gruppe war nicht signifikant. Bei Hinzunahme von weiteren rekrutierten Parametern (Body-Mass-Index, Frailty, Sturzanamnese, Gehhilfe, CHA2DS2-VASc-Score, C-reaktives Protein, Hämoglobin) konnte kein signifikanter Unterschied zwischen der Neurologie- und Psychiatrie-Gruppe mehr gefunden werden.
Das Risiko sowie die Krankheitsschwere einer Parkinson-Erkrankung, gemessen am Hoehn-Yahr-Stadium und den erreichten Werten im MDS UPDRS III, korrelierten mit dem Vitamin D-Serumspiegel. Allerdings war auch hier nach Hinzunahme von Kovariaten wie Alter, Geschlecht und Krankheitsdauer der Effekt nicht mehr signifikant.
Die Ergebnisse unterstützen die Annahme, dass die vorgefundenen niedrigen 25-(OH)-Vit D-Serumspiegel bei Parkinson-Patienten ein krankheitsbegleitendes Phänomen ist, das womöglich durch die eingeschränkten motorischen Fähigkeiten mit resultierend niedriger Sonnenexposition bedingt ist und durch zunehmende Kranheitsdauer und damit Krankheitsschwere verstärkt wird. Da es sich jedoch beim IPS um eine Krankheit handelt, die zum Einen mit motorischen Einschränkungen und resultierend erhöhtem Sturzrisiko einhergeht und zum Anderen vorwiegend Menschen höheren Alters betrifft, besteht ein erhöhtes Osteoporose- und sturzbedingtes Frakturrisiko, sodass ein Monitoring des Vitamin D-Serumspiegels sowie eine gegebenenfalls notwendige Vitamin D-Supplementierung weiterhin eine Rolle in der Behandlung von Parkinson-Patienten spielen.
Background: Recently, attention has grown toward cerebellar neuromodulation in motor learning using transcranial direct current stimulation (tDCS). An important point of discussion regarding this modulation is the optimal timing of tDCS, as this parameter could significantly influence the outcome. Hence, this study aimed to investigate the effects of the timing of cerebellar anodal tDCS (ca-tDCS) on motor learning using a sequential finger-tapping task (FTT).
Methods: One hundred and twenty two healthy young, right-handed subjects (96 females) were randomized into four groups (During\(_{sham}\), Before, During\(_{real}\), After). They performed 2 days of FTT with their non-dominant hand on a custom keyboard. The task consisted of 40 s of typing followed by 20 s rest. Each participant received ca-tDCS (2 mA, sponge electrodes of 25 cm\(^{2}\), 20 min) at the appropriate timing and performed 20 trials on the first day (T1, 20 min). On the following day, only 10 trials of FTT were performed without tDCS (T2, 10 min). Motor skill performance and retention were assessed.
Results: All participants showed a time-dependent increase in learning. Motor performance was not different between groups at the end of T1 (p = 0.59). ca-tDCS did not facilitate the retention of the motor skill in the FTT at T2 (p = 0.27). Thus, our findings indicate an absence of the effect of ca-tDCS on motor performance or retention of the FTT independently from the timing of stimulation.
Conclusion: The present results suggest that the outcome of ca-tDCS is highly dependent on the task and stimulation parameters. Future studies need to establish a clear basis for the successful and reproducible clinical application of ca-tDCS.
Background: Multiple sclerosis (MS) may cause variable functional impairment. The discrepancy between functional impairment and brain imaging findings in patients with MS (PwMS) might be attributed to differential adaptive and consolidation capacities. Modulating those abilities could contribute to a favorable clinical course of the disease.
Objectives: We examined the effect of cerebellar transcranial direct current stimulation (c-tDCS) on locomotor adaptation and consolidation in PwMS using a split-belt treadmill (SBT) paradigm.
Methods: 40 PwMS and 30 matched healthy controls performed a locomotor adaptation task on a SBT. First, we assessed locomotor adaptation in PwMS. In a second investigation, this training was followed by cerebellar anodal tDCS applied immediately after the task ipsilateral to the fast leg (T0). The SBT paradigm was repeated 24 h (T1) and 78 h (T2) post-stimulation to evaluate consolidation.
Results: The gait dynamics and adaptation on the SBT were comparable between PwMS and controls. We found no effects of offline cerebellar anodal tDCS on locomotor adaptation and consolidation. Participants who received the active stimulation showed the same retention index than sham-stimulated subjects at T1 (p = 0.33) and T2 (p = 0.46).
Conclusion: Locomotor adaptation is preserved in people with mild-to-moderate MS. However, cerebellar anodal tDCS applied immediately post-training does not further enhance this ability. Future studies should define the neurobiological substrates of maintained plasticity in PwMS and how these substrates can be manipulated to improve compensation. Systematic assessments of methodological variables for cerebellar tDCS are urgently needed to increase the consistency and replicability of the results across experiments in various settings.
The objective of this study was to examine the therapeutic potential of multiple sessions of training on a split-belt treadmill (SBT) combined with cerebellar anodal transcranial direct current stimulation (tDCS) on gait and balance in People with Multiple Sclerosis (PwMS). Twenty-two PwMS received six sessions of anodal (PwMS\(_{real}\), n = 12) or sham (PwMS\(_{sham}\), n = 10) tDCS to the cerebellum prior to performing the locomotor adaptation task on the SBT. To evaluate the effect of the intervention, functional gait assessment (FGA) scores and distance walked in 2 min (2MWT) were measured at the baseline (T0), day 6 (T5), and at the 4-week follow up (T6). Locomotor performance and changes of motor outcomes were similar in PwMS\(_{real}\) and PwMS\(_{sham}\) independently from tDCS mode applied to the cerebellum (anodal vs. sham, on FGA, p = 0.23; and 2MWT, p = 0.49). When the data were pooled across the groups to investigate the effects of multiple sessions of SBT training alone, significant improvement of gait and balance was found on T5 and T6, respectively, relative to baseline (FGA, p < 0.001 for both time points). The FGA change at T6 was significantly higher than at T5 (p = 0.01) underlining a long-lasting improvement. An improvement of the distance walked during the 2MWT was also observed on T5 and T6 relative to T0 (p = 0.002). Multiple sessions of SBT training resulted in a lasting improvement of gait stability and endurance, thus potentially reducing the risk of fall as measured by FGA and 2MWT. Application of cerebellar tDCS during SBT walking had no additional effect on locomotor outcomes.
Die Pathomechanismen der Neuropathie bei Neuroborreliose sind noch immer unklar. In der vorliegenden Studie untersuchten wir 22 Patienten mit einer Neuropathie bei Neuroborreliose (BN) (3 Patienten in Stadium 2, 19 Patienten in Stadium 3) und verglichen diese mit 9 Patienten einer vaskulitischen Neuropathie (VN) und 14 Patienten einer idiopathisch axonalen Neuropathie (AN). Histologische und immunhistochemische Färbungen wurden mit Antikörpern gegen Leu4, CD68, 27E10 (frühzeitig aktivierte Makrophagen) und 25F9 (späte Makrophagen), Membrane-Attack-Komplex C5b-9, Adhäsionsmolekül ICAM sowie inflammatorische Zytokine Tumor Nekrose-Faktor-alpha (TNF-alpha), Interleukin-1ß (IL-1ß) und Interleukin-6 (IL-6), als auch Metalloproteinase MMP-9 durchgeführt, ferner mit Antikörpern gegen das membranassoziierte Glykoprotein HLA-DR3. Zusätzlich wurden Semi-Dünn-Schnitte angefertigt. Klinisch wiesen die meisten BN Patienten eine distal symmetrische sensomotorische Neuropathie auf, nur 6/22 Patienten waren schmerzfrei. Die Mehrzahl (18/22) der BN Neuropathien waren primär axonal mit perivaskulären (6/22), in 8 Fällen vaskulitischen Infiltraten. Das Perineurium war schwerpunktmäßig im Rahmen einer Borrelien-assoziierten Neuropathie betroffen. Das lies sich aus dem gegenüber den Kontrollgruppen signifikant verdickten Perineurium, der vermehrten Vaskularisation des Perineuriums und der starken IR für das Zytokin TNF-α schliessen, in geringerem Ausmaß für IL-1β, und für die terminale Komplementkomponente C5b9. Perivaskuläre und vaskuläre Infiltrate sowie die betont perineurale Expression bestimmter inflammatorischer Zytokine und Adhäsionsmoleküle erschienen charakteristisch für eine Neuropathie bei Neuroborreliose. Autoimmune Reaktionen mit Angriff am Perineurium können für die Pathogenese der Neuropathie bei Neuroborreliose bedeutsam sein.
Die Chronische Inflammatorische Demyelinisierende Polyneuropathie (CIDP) ist eine seltene autoimmune Erkrankung des peripheren Nervensystems. In der vorliegenden Dissertation wurden die Akten von 23 CIDP – Patienten der Neurologischen Klinik der Universität Würzburg aus dem Zeitraum von 1990 bis 1999 ausgewertet und die elektrophysiologischen Parameter sowohl mit den klinischen Befunden als auch mit den Biopsiebefunden in Beziehung gestellt. Folgende Schlussfolgerungen konnten gezogen werden : - In dem untersuchten Patientenkollektiv war die sensomotorische Form der CIDP mit überwiegend motorischen Symptomen die häufigste Ausprägungsform. Hingegen treten rein motorische und rein sensible Formen sehr selten auf. - Die Krankheit manifestierte sich überwiegend an den distalen Extremitäten. - Die oberen Extremitäten waren selten und in keinem Fall isoliert betroffen. - Die systematische Auswertung des klinischen Schwergrades erfolgte anhand des Modifizierten Rankin Scores, der sowohl motorische als auch sensible Symptome berücksichtigt. Zusätzlich erfolgte der Entwurf eines sensiblen Scores, bei dem die sensiblen Symptome der Patienten in Zahlenwerten ausgedrückt sind. - Der Vergleich des klinischen Schweregrades mit den neurophysiologischen Befunden ergab vier unterschiedliche Korrelationstypen, die von sehr enger Korrelation bis hin zu nur geringen Übereinstimmungen reichten. - Der Schwergrad der Beeinträchtigung der Patienten war im Langzeitverlauf mit dem axonalen Verlust korreliert. Es fand sich eine gute Korrelation der Muskelsummenaktionspotentiale (CMAP) des Nervus medianus und des Nervus tibialis sowie der sensiblen Nervenaktionspotentiale (SNAP) des Nervus suralis mit dem Score. - Es ergab sich ein signifikanter Zusammenhang zwischen der Amplitude des Nervus suralis NAP`s und dem Ausmaß des Axonverlustes, wohingegen zwischen der Nervenleitungsgeschwindigkeit des Nervus suralis und dem Axonverlust nur eine geringe Abhängigkeit besteht. - Der Grad der Demyelinisierung korrelierte mit der Anzahl der endoneuralen Makrophagen. - Zwischen der Nervenleitgeschwindigkeit des Nervus suralis und dem Grad der Demyelinisierung im Biopsat konnte keine eindeutige Abhängigkeit nachgewiesen werden. - Es besteht ein Zusammenhang zwischen der Spontanaktivität im EMG und im Verlauf abnehmenden Nervenleitgeschwindigkeiten des Nervus medianus, der aber statistisch nicht signifikant ist. - Insbesondere ist die NLG- Abnahme in den ersten Wochen ein prognostisch ungünstiges Zeichen. - Eine Analogie zwischen einer durch das EMG nachgewiesenen Spontanaktivität und der Amplitude des Nervus suralis besteht. - Zwischen den Amplituden der motorischen Summenaktionspotentiale des Nervus tibialis oder den Amplituden der sensiblen Nervenaktionspotentiale des Nervus suralis auf der einen Seite und dem Ausmaß der Spontanaktivität im EMG auf der anderen Seite bestehen erkennbare Korrelationen.
Die Multiple Sklerose ist eine bisher nicht heilbare, chronisch-inflammatorische demyelinisierende Erkrankung des zentralen Nervensystems. Trotz intensiver Forschungsbemühungen ist der exakte Pathomechanismus nicht vollkommen verstanden. Klar ist jedoch, dass der Blut-Hirn-Schranke eine entscheidende Rolle bei der Pathogenese zukommt. Seit Februar 2014 ist mit Dimethylfumarat ein neues orales Medikament für die schubförmige Multiple Sklerose zugelassen. Die Wirkungen von Fumarsäureestern auf humane zerebrale Endothelzellen als Grundsteine der Blut-Hirn-Schranke sind allerdings nur unzureichend untersucht.
Mehrere Forschungsgruppen demonstrierten an humanem Nabelschnurvenenendothel einen hemmenden Effekt von Fumarsäureestern auf die Adhäsion von Leukozyten und beschrieben eine Inhibition der Aktivierung des proinflammatorischen Transkriptionsfaktors NFB in den Endothelzellen. Aufgrund der charakteristischen Eigenschaften zerebralen Endothels ist eine Übertragung dieser Beobachtungen auf die Blut-Hirn-Schranke allerdings nicht ohne weiteres möglich. Daher galt es potentielle Effekte von Fumarsäureestern auf primäre humane zerebrale Endothelzellen als in vitro Modell der Blut-Hirn-Schranke zu überprüfen. Dabei wurden die Zellen nicht nur unter ruhenden Bedingungen, sondern auch unter inflammatorischer Stimulation mit TNF-α, IL-1 und IFN untersucht, einem Milieu, wie es in inflammatorischen MS Läsionen zu finden ist. In Leukozyten-Adhäsionsassays konnte durch Inkubation mit Monomethylfumarat und Dimethylfumarat keine funktionale Beeinflussung der Adhäsion von T-Lymphozyten an den verwendeten zerebralen Endothelzellen verzeichnet werden. Kongruent dazu fand sich in durchflusszytometrischen Analysen keine Hemmung der inflammatorisch vermittelten Expression des Adhäsionsmoleküls ICAM-1, welches eine tragende Rolle bei der Leukozytenmigration spielt. Inflammatorische intrazelluläre Signalwege, wie die NFB-Kerntranslokation oder die Phosphorylierung von p38 wurden in HECE im Gegensatz zu HUVEC durch Fumarsäureester ebenso wenig beeinflusst.
Diese in sich konsistenten Ergebnisse führen zu der Schlussfolgerung, dass im Gegensatz zu anderen Gefäßbetten weder Dimethylfumarat noch Monomethylfumarat direkt am zerebralen Endothel anti-inflammatorisch wirken.
Der Verlauf der Multiplen Sklerose ist heterogener Natur; die Fähigkeit zu einem intakten adaptiven motorischen Lernen und einer intakten Konsolidierung könnten einen milden Krankheitsverlauf begünstigen.
In der vorliegenden Arbeit wurden das adaptive motorische Lernen und seine Konsolidierung bei MS-Patienten im Vergleich zu neurologisch gesunden Kontrollprobanden untersucht; außerdem wurde das Verhältnis dieser Formen des Lernens zu klinischen und apparativen Parametern des Krankheitsprogresses untersucht.
Dazu führten 20 MS-Patienten und 20 Kontrollprobanden eine visuoadaptive Lernaufgabe durch. Hierzu sollten mittels Computerbildschirm und Computermaus geradlinige Zielbewegungen zwischen einem Startpunkt und einem Zielpunkt wechselnder Lokalisation durchgeführt werden, wobei in einem Rotationsmodus eine externe Ablenkung der Zielbewegung im Uhrzeigersinn eingeführt wurde, welche auszugleichen war. Die Übungssitzung wurde nach 24 Stunden und nach 72 Stunden wiederholt. Analysiert wurden die Richtungsfehler der Zielbewegungen, die Adaptationsrate an die Ablenkung und die Retention der erlernten Adaptation bis zur Folgesitzung. Motorische Einschränkung wurde durch den EDSS-Score und den 9-Loch-Stecktest quantifiziert, zentralnervöse Läsionslast wurde mittels cMRT und MEP ermittelt.
Die Adaptation und Lernfähigkeit innerhalb einer Übungssitzung waren in der Patienten- und der Kontrollgruppe vergleichbar; jedoch zeigte sich eine signifikant verminderte Retentionsrate in der Patientengruppe an den Folgeuntersuchungstagen im Vergleich zur Kontrollgruppe. In den Korrelationsanalysen und Subgruppenvergleichen innerhalb der Patientengruppe nach Stratifizierung aufgrund von EDSS-Score, 9-Lochstecktest und zentralnervöser Läsionslast im MRT konnte kein eindeutiger Zusammenhang zwischen klinischer Beeinträchtigung bzw. zentralnervöser Läsionslast auf der einen Seite und Adaptation bzw. Konsolidierung auf der anderen Seite identifiziert werden. Jedoch zeigte sich in der Patientengruppe für den ersten Nachuntersuchungstag eine signifikant höhere Retentionsrate in der Subgruppe mit geringerer Leistung im 9-Lochsteck-Test.
Insgesamt deuten die vorliegenden Daten auf eine erhaltene Fähigkeit zu adaptivem motorischen Lernen und somit auf eine erhaltene rasch einsetzende Neuroplastizität bei leicht bis mittelgradig betroffenen MS-Patienten hin; jedoch sprechen die Daten für eine eingeschränkte Konsolidierungsfähigkeit. Zentralnervöse Läsionslast scheint Motoradaptation und Konsolidierung nicht zu verhindern. Das genaue Verhältnis der Motoradapation und Konsolidierung zum klinischen Funktionserhalt konnte nicht genauer aufgeklärt werden. Um die genaue Beziehung zwischen Motoradaptation und Konsolidierung und klinischer Beeinträchtigung bzw. ZNS-Läsionen zu eruieren, bedarf es weiterer Studien.
We conducted a genome-wide association study of essential tremor, a common movement disorder characterized mainly by a postural and kinetic tremor of the upper extremities. Twin and family history studies show a high heritability for essential tremor. The molecular genetic determinants of essential tremor are unknown. We included 2807 patients and 6441 controls of European descent in our two-stage genome-wide association study. The 59 most significantly disease-associated markers of the discovery stage were genotyped in the replication stage. After Bonferroni correction two markers, one (rs10937625) located in the serine/threonine kinase STK32B and one (rs17590046) in the transcriptional coactivator PPARGC1A were associated with essential tremor. Three markers (rs12764057, rs10822974, rs7903491) in the cell-adhesion molecule CTNNA3 were significant in the combined analysis of both stages. The expression of STK32B was increased in the cerebellar cortex of patients and expression quantitative trait loci database mining showed association between the protective minor allele of rs10937625 and reduced expression in cerebellar cortex. We found no expression differences related to disease status or marker genotype for the other two genes. Replication of two lead single nucleotide polymorphisms of previous small genome-wide association studies (rs3794087 in SLC1A2, rs9652490 in LINGO1) did not confirm the association with essential tremor.
Hereditäre periphere Neuropathien sind chronische Erkrankungen des peripheren Nervensystems, einhergehend mit Muskelschwäche und sensorischer Dysfunktion. Die Merkelzelle als Mechanorezeptor der Haut wird von einer myelinisierten langsam adaptierenden Afferenz (Aß) innerviert. Deshalb wurde in der vorliegenden Arbeit als sekundärer Indikator für die distale Innervation in P0-defizienten Mäusen, ein Tiermodell der hereditären motorisch-sensorischen Déjérine-Sottas-Neuropathie, die Merkelzellzahl in der Haut dieser Tiere untersucht. Zusätzlich wurde untersucht, ob durch den Gendefekt das unmyelinisierte Nervenfasersystem der Haut und das Vorkommen von Neurotrophinen (NT-3, NGF) in den P0-defizienten Mäusen beeinflußt wird. Zur Anwendung kamen 6 Wochen, 4 Monate und 6 Monate alte Wildtyp- und P0-defiziente Mäuse. Unter Zuhilfenahme immunhistochemischer Färbemethoden, computergestützter und lichtmikroskopischer Auswerteverfahren konnte ein Verlust von Merkelzellen in behaarter als auch in unbehaarter Haut mit fortschreitenden Alter der Tiere gezeigt werden. Die Bestimmung des NT-3- und NGF-Gehaltes mittels enzymgekoppelten Assays ergab keine signifikante Reduktion des Vorkommens dieser Neurotrophine. Neben dem beträchtlichen Verlust an Merkelzellen konnte man keinen Effekt auf unmyelinisierte intraepidermale Nervenfasern nachweisen. Im Gegensatz dazu stellte eine parallel durchgeführte Studie an Nervus femoralis und Nervus ischiadicus in 6 Monate alten P0-defizienten Tieren eine 70 Prozent Reduktion myelinisierter Axone fest, so daß wir daraus eine Abhängigkeit der Merkelzelle von ihrer assoziierten myelinisierten SAI-Afferenz sowohl in behaarter als auch in unbehaarter Haut in den P0-defizienten Tieren schlußfolgerten. Der Verlust an Merkelzellen und die Beeinträchtigung von myelinisierten Nervenfasern in den P0-defizienten Tieren kann nicht als sekundäre Wirkung eines veränderten trophischen Gehaltes an NT-3 oder NGF angesehen werden, sondern ist vielmehr durch den axonalen Verlust erklärbar. Die intraepidermalen unmyelinisierten Nervenfasern bleiben durch den Gendefekt unbeeinflußt, so daß die Mutation im P0-Gen eine Spezifität für das myelinisierte Nervenfasersystem zeigt. Wichtige Ergebnisse dieser Arbeit wurden bereits veröffentlicht.
In den letzten Jahren wurden große Fortschritte in der Aufklärung von Mechanismen der Protonensensibilität auf molekularer Ebene gemacht, die ein wesentliches Element der Nozizeption darstellt. In dieser Arbeit wurde der Einfluss von den in diesem Zusammenhang entdeckten Kanälen TRPV1 und ASIC3 auf die Protonensensitivität von nativen Spinalganglienneuronen, sowie Unterschiede zwischen der IB4-positiven und der IB4-negativen Population untersucht. Hierzu wurden Patch-Clamp-Studien an isolierten Neuronen von TRPV1-defizienten und ASIC3-defizienten Mäusen durchgeführt. Die Ergebnisse dieser Experimente bestätigen dabei die wesentliche Rolle von TRPV1 für die Protonensensibilität. Insbesondere für nicht desensibilisierende Komponenten von protoneninduzierten Strömen, die für die Transduktion extrazelluärer azidotischer Schmerzzustände in eine anhaltende Erregung des nozizeptiven Systems verantwortlich sind, ist TRPV1 von großer Bedeutung. Diese sind in den TRPV1-defizienten Neuronen stark reduziert. Der Einfluss von ASIC3 auf diese Komponenten ist hingegen gering, auch wenn es Hinweise auf eine Beteiligung dieses Rezeptors an diesen Komponenten gibt. Größere Bedeutung hat ASIC3 für schnell desensibilisierende Komponenten der Reaktion dieser Neurone auf Protonen, die von ASIC3-defizienten seltener als von Wildtyp-Neuronen gezeigt werden. Die Bedeutung dieser transienten Komponenten ist nicht geklärt, wahrscheinlich erfüllen sie eine modulatorische Funktion, nicht nur im nozizeptiven System. Noch wenig ist über die funktionellen Unterschiede der zwei verschiedenen Subpopulationen nozizeptiver Neurone bekannt, die durch die Bindung des Isolektins B4 differenziert werden können. Diese Arbeit gibt Hinweise darauf, dass sich diese auch in ihrer Protonensensitivität unterscheiden. Das könnte Ausdruck dafür sei, dass diese an der Wahrnehmung unterschiedlicher Schmerzzustände beteiligt sind. Die Charakterisierung der Mechanismen des komplexen nozizeptiven Systems auf zellulärer und molekularer Ebene ist Vorraussetzung zur Entwicklung von gezielt wirkenden, analgetischen Pharmaka. Die schon lange bekannte Wirksamkeit von Capsaicin – dem wohl bekanntesten Agonisten von TRPV1 – bei verschiedenen schmerzhaften Zuständen und fortschreitende Erkenntnisse über die Bedeutung dieses und der ASIC-Rezeptoren bei der Wahrnehmung von schmerzassoziierter Gewebsazidose, zeigt Wege auf, über die solche Medikamente ihre Wirkung entfalten könnten.
Die hereditäre Spinalparalyse SPG17 ist eine autosomal-dominant vererbte Motoneuronerkrankung, welche durch Mutationen im BSCL2 (Seipin) Gen verursacht wird. Klassischerweise äußert sich die Krankheit durch eine spastische Paraparese der Beine und Amyotrophie der Hände (Silver-Syndrom) oder eine vorwiegend periphere (senso-)motorische Neuropathie. Für die vorliegende Arbeit wurden insgesamt sieben Patienten aus vier verschiedenen Familien, bei denen heterozygote Mutationen im BSCL2 Gen nachgewiesen werden konnten, klinisch sowie elektrophysiologisch und molekulargenetisch untersucht. Es gelang hierbei zwei bisher unbekannte phänotypische Ausprägungen zu beschreiben, welche die Symptomatik und den Verlauf einer Multifokalen Motorischen Neuropathie (MMN) bzw. einer Amyotrophen Lateralsklerose (ALS) imitieren und hiervon nur durch den genetischen Befund zu unterscheiden sind. Anhand dieser Ergebnisse erfolgte dann nach extensiver Literaturrecherche eine Zusammenfassung aller bisher publizierten Fälle der SPG17 und eine Einordnung der hier erstbeschriebenen Phänotypen in einen Vorschlag zur Erweiterung des bisher verwendeten Klassifikationssystems von BSCL 2 Mutationen.
Der Einfluß von Virusinfektion und Impfung auf autoreaktive T-Lymphozyten bei der Multiplen Sklerose

In der sogenannten ViMS-Studie, bei der MS-Patienten und gesunde Kontrollpersonen mit einer Influenza-Spaltvakzine geimpft und für einen zum Teil viermonatigen Zeitraum im Verlauf nachbeobachtet wurden, ergab sich weder mit dem sensitiven IFNg-ELISPOT noch mit der quantitativen RT-PCR ein Anhalt für erhöhte Autoimmunreaktivität gegen die zwei untersuchten Myelin-Antigene MBP und MOG. Im Gegensatz dazu konnten mit dem IFNg-ELISPOT-Assay bei einigen gesunden Spendern und MS-Patienten nach natürlichen Atemwegsinfektionen eine erhöhte Frequenz autoreaktiver MBP-spezifischer T-Lymphozyten beobachtet werden. Im zweiten Teil dieser Arbeit konnten durch Zellkulturinfektionen mit Influenzavirus oder HHV-6 weder an Primärzellkulturen noch in einem etablierten in vitro-Modell für MS-Autoimmunität an MBP-spezifischen T-Zellen eine immunstimulierende Wirkung gezeigt werden. Bei niedrigen Infektionsdosen kam es zur Proliferation einer wahrscheinlich virus-spezifischen Zellpopulation, bei höheren Dosen wurde dieser Effekt durch die bekannte Immunsuppression der in vitro-Infektion mit HHV-6 übertroffen. In einer umfassenden Untersuchung von Serumproben von gesunden Spendern und MS-Patienten in unterschiedlichen Krankheitsphasen wurden trotz sensitiver Nachweismethoden keine erhöhten Antikörper-Titer (IgG/IgM) gegen HHV-6 oder HHV-6-DNA nachgewiesen, woraus geschlossen werden darf, daß die untersuchten Viren keine intrinsische Pathogenität für die Entstehung von Autoimmunität bei der MS aufweisen. Im Vergleich zu der Kontrollgruppe erhöhte Anti-HHV-6-IgG-Titer bei PTX-behandelten MS-Patienten lassen sich als mögliches Epiphänomen durch die immun-modulatorische (Th2-vermittelte) Wirkung des Medikaments deuten. In Zusammenschau aller Ergebnisse dieser Arbeit lassen sich die anfangs angedeuteten Modelle einer virusvermittelten Autoimmunpathogenese der MS nicht eindeutig ein-ordnen. Die Ergebnisse der ViMS-Studie, unterstützt durch zahlreiche Untersuchungen anderer Gruppen, weisen in Bezug auf Schubauslösung oder Verschlechterung auf einen generellen immunaktivierenden Mechanismus im Sinne einer unspezifischen Begleitreaktion durch Infektion aber nicht durch Influenzaschutzimpfung hin. Dabei spielt wohl nicht eine einzelne Virusinfektion die maßgebliche Rolle in einem schon auf immunologischer Ebene recht komplexen Netzwerk, sondern können prinzipiell verschiedene (beliebige) Viren zum Anstoßen einer Autoimmunkaskade beitragen, wenn sie auf einen konstitutionell oder temporär empfänglichen Wirtsorganismus treffen. Dies ist auch vom Infektionsort und –milieu abhängig. Bei der vorliegenden Multifaktorialität und Heterogenität der Subpopulatio-nen sind monolineare Erklärungsansätze bislang zum Scheitern verurteilt gewesen. Aber aus dem Fehlen eines Beweises kann nicht der Beweis für das Fehlen eines Zusammen-hangs zwischen Virusinfektionen und Autoimmunreaktionen geschlossen werden.
Background
Troponin elevation is common in ischemic stroke (IS) patients. The pathomechanisms involved are incompletely understood and comprise coronary and non-coronary causes, e.g. autonomic dysfunction. We investigated determinants of troponin elevation in acute IS patients including markers of autonomic dysfunction, assessed by heart rate variability (HRV) time domain variables.
Methods
Data were collected within the Stroke Induced Cardiac FAILure (SICFAIL) cohort study. IS patients admitted to the Department of Neurology, Würzburg University Hospital, underwent baseline investigation including cardiac history, physical examination, echocardiography, and blood sampling. Four HRV time domain variables were calculated in patients undergoing electrocardiographic Holter monitoring. Multivariable logistic regression with corresponding odds ratios (OR) and 95% confidence intervals (CI) was used to investigate the determinants of high-sensitive troponin T (hs-TnT) levels ≥14 ng/L.
Results
We report results from 543 IS patients recruited between 01/2014–02/2017. Of those, 203 (37%) had hs-TnT ≥14 ng/L, which was independently associated with older age (OR per year 1.05; 95% CI 1.02–1.08), male sex (OR 2.65; 95% CI 1.54–4.58), decreasing estimated glomerular filtration rate (OR per 10 mL/min/1.73 m2 0.71; 95% CI 0.61–0.84), systolic dysfunction (OR 2.79; 95% CI 1.22–6.37), diastolic dysfunction (OR 2.29; 95% CI 1.29–4.02), atrial fibrillation (OR 2.30; 95% CI 1.25–4.23), and increasing levels of C-reactive protein (OR 1.48 per log unit; 95% CI 1.22–1.79). We did not identify an independent association of troponin elevation with the investigated HRV variables.
Conclusion
Cardiac dysfunction and elevated C-reactive protein, but not a reduced HRV as surrogate of autonomic dysfunction, were associated with increased hs-TnT levels in IS patients independent of established cardiovascular risk factors.
Neuroprotection aims to prevent salvageable neurons from dying. Despite showing efficacy in experimental stroke studies, the concept of neuroprotection has failed in clinical trials. Reasons for the translational difficulties include a lack of methodological agreement between preclinical and clinical studies and the heterogeneity of stroke in humans compared to homogeneous strokes in animal models. Even when the international recommendations for preclinical stroke research, the Stroke Academic Industry Roundtable (STAIR) criteria, were followed, we have still seen limited success in the clinic, examples being NXY-059 and haematopoietic growth factors which fulfilled nearly all the STAIR criteria. However, there are a number of neuroprotective treatments under investigation in clinical trials such as hypothermia and ebselen. Moreover, promising neuroprotective treatments based on a deeper understanding of the complex pathophysiology of ischemic stroke such as inhibitors of NADPH oxidases and PSD-95 are currently evaluated in preclinical studies. Further concepts to improve translation include the investigation of neuroprotectants in multicenter preclinical Phase III-type studies, improved animal models, and close alignment between clinical trial and preclinical methodologies. Future successful translation will require both new concepts for preclinical testing and innovative approaches based on mechanistic insights into the ischemic cascade.
Movement preparation and bilateral modulation of beta activity in aging and Parkinson's disease

In previous studies of young subjects performing a reaction-time reaching task, we found that faster reaction times are associated with increased suppression of beta power over primary sensorimotor areas just before target presentation. Here we ascertain whether such beta decrease similarly occurs in normally aging subjects and also in patients with Parkinson's disease (PD), where deficits in movement execution and abnormalities of beta power are usually present. We found that in both groups, beta power decreased during the motor task in the electrodes over the two primary sensorimotor areas. However, before target presentation, beta decreases in PD were significantly smaller over the right than over the left areas, while they were symmetrical in controls. In both groups, functional connectivity between the two regions, measured with imaginary coherence, increased before the target appearance; however, in PD, it decreased immediately after, while in controls, it remained elevated throughout motor planning. As in previous studies with young subjects, the degree of beta power before target appearance correlated with reaction time. The values of coherence during motor planning, instead, correlated with movement time, peak velocity and acceleration. We conclude that planning of prompt and fast movements partially depends on coordinated beta activity of both sensorimotor areas, already at the time of target presentation. The delayed onset of beta decreases over the right region observed in PD is possibly related to a decreased functional connectivity between the two areas, and this might account for deficits in force programming, movement duration and velocity modulation.
FoxP3+Regulatory T Cells Determine Disease Severity in Rodent Models of Inflammatory Neuropathies

Inflammatory neuropathies represent disabling human autoimmune disorders with considerable disease variability. Animal models provide insights into defined aspects of their disease pathogenesis. Forkhead box P3 (FoxP3)+ regulatory T lymphocytes (Treg) are anti-inflammatory cells that maintain immune tolerance and counteract tissue damage in a variety of immune-mediated disorders. Dysfunction or a reduced frequency of Tregs have been associated with different human autoimmune disorders. We here analyzed the functional relevance of Tregs in determining disease manifestation and severity in murine models of autoimmune neuropathies. We took advantage of the DEREG mouse system allowing depletion of Treg with high specificity as well as anti-CD25 directed antibodies to deplete Tregs in mice in actively induced experimental autoimmune neuritis (EAN). Furthermore antibody-depletion was performed in an adoptive transfer model of chronic neuritis. Early Treg depletion increased clinical EAN severity both in active and adoptive transfer chronic neuritis. This was accompanied by increased proliferation of myelin specific T cells and histological signs of peripheral nerve inflammation. Late stage Treg depletion after initial disease manifestation however did not exacerbate inflammatory neuropathy symptoms further. We conclude that Tregs determine disease severity in experimental autoimmune neuropathies during the initial priming phase, but have no major disease modifying function after disease manifestation. Potential future therapeutic approaches targeting Tregs should thus be performed early in inflammatory neuropathies.
Die Literatur beschreibt unter Patienten mit idiopathischer Small fiber Neuro-pathie (SFN) einen Anteil von etwa ein Fünftel bis Drittel mit Variationen unkla-rer pathogenetischer Relevanz in Schmerz-assoziierten Genen. Dies bestätig-te sich im Rahmen unserer klinischen Studie: Über die Zeit von Mai 2015 bis Januar 2020 konnten bei 13 von 66 (21%) eingeschlossenen Patienten mit klinischem Verdacht auf SFN genetische Variationen in Schmerz-assoziierten Genen detektiert werden. Solche Veränderungen können über Gain- oder Loss-of-Function-Mechanismen die Funktion codierter nozizeptiver Signalpro-teine modulieren und so potentiell zur SFN-Symptomatik führen. Im Rahmen der Studie erfolgte neben der genetischen Diagnostik eine umfangreiche Un-tersuchung der Teilnehmer. In der Diagnostik stachen die potentiell geneti-schen SFN-Patienten nicht heraus und auch klinisch fielen nur dezente Unter-schiede zu den übrigen Patienten auf: Wir zeigten, dass die Betroffenen häufi-ger von äußeren Einflussfaktoren getriggerte Schmerzattacken erleiden und eine tendenziell weitläufigere Symptommanifestation aufwiesen. Dies ähnelt der Klinik anderer hereditärer neuropathischer Schmerzsyndrome wie der pa-roxysmalen extremen Schmerzstörung (PEPD) oder den familiären episodi-schen Schmerzsyndromen (FEPS). Die potentiell genetische Grundlage führte bei unseren Patienten zu einer stärkeren Limitation im täglichen und berufli-chen Alltag und minderte die Lebensqualität der Betroffenen deutlich. Mögli-che Ursache hierfür war auch die herausfordernde Therapie der Patienten mit Genvariationen: Für den gleichen Behandlungserfolg mussten die Patienten mit potentiell genetischer SFN deutlich mehr Wirkstoffe einnehmen und über-haupt versuchen. Obwohl nur minimale klinische Hinweise eine potentiell ge-netische Genese andeuten, sollten diese frühzeitig durch eine strukturierte Anamnese erkannt werden. Die Sammlung von Daten zu betroffenen Familien kann die pathogenetische Relevanz der Variationen erhärten. Auch wird im Feld der genetischen Schmerzforschung rasant an zielgerichteten Analgetika gearbeitet, die fehlregulierte Rezeptoren blockieren sollen. Damit könnte Be-troffenen künftig gezielt geholfen werden. Wir empfehlen auf Grundlage unse-rer Studie bei Vorliegen genannter hinweisender Charakteristika eine geneti-sche Testung und Beratung zusätzlich zur weiteren ätiologischen Diagnostik. Das zu untersuchende Panel sollte möglichst viele Schmerz-assoziierte Gene umfassen – vorrangig die Gene codierend für die spannungsabhängigen Nat-riumkanäle SCN9A, -10A und 11A und die TRP-Kanalproteine TRPA1, TRPV1 und -3.
Autoantikörper gegen nodo-paranodale Proteine des Ranvier’schen Schnürrings wie
Neurofascin-155 (NF-155), Contactin-1 und Caspr wurden in der Literatur bei
Patienten/Patientinnen mit Immunneuropathien beschrieben. Bei zwei bis zehn Prozent
der Patienten/Patientinnen mit Immunneuropathien können Autoantikörper gegen
Isoformen des Neurofascin detektiert werden. Patienten/Patientinnen mit
Autoantikörpern gegen NF-155 weisen gemeinsame klinische Merkmale auf, unter
anderem einen schweren Verlauf mit subakutem Beginn, vorwiegend motorischen
Defiziten, Tremor und einem schlechten Ansprechen auf eine Therapie mit intravenösen
Immunglobulinen (IVIG). Ein Grund für Letzteres könnte sein, dass es sich überwiegend
um Autoantikörper der Subklasse IgG4 handelt, die als anti-inflammatorisch gelten und
kein Komplement aktivieren. Neben der IgG4-Subklasse können bei manchen
Erkrankten auch die proinflammatorischen IgG-Subklassen 1 bis 3 nachgewiesen
werden. Bei der Anti-Pan-Neurofascin (155/140/186) Polyneuropathie zeigt sich klinisch
häufig ein fulminanter Phänotyp mit IgG3 Prädominanz. Das Ziel dieser Studie war, die
Autoantikörper-induzierte Komplementablagerung zu detektieren, sowie die Rolle der
IgG Subklasse und die Effekte von IVIG auf Antikörperbindung, Komplementaktivierung
und Effektorfunktionen zu untersuchen.
Hierzu wurde das Serum von 212 Probanden/-innen mit der Verdachtsdiagnose einer
entzündlichen Neuropathie auf Autoantikörper gegen NF-155 mittels ELISA und
Bindungsversuchen an Mäusezupfnerven gescreent. Im Fall eines positiven
Ergebnisses dienten zellbasierte Bindungsversuche mit NF-155-transfizierten HEK-293-
Zellen als Bestätigungstest. Die Effekte unterschiedlicher IVIG Konzentrationen auf die
Antikörperbindung und Komplementablagerung wurden in ELISA,
Komplementbindungsassays und zellbasierten Verfahren getestet. Außerdem wurde
mithilfe von LDH-Zytotoxizitätsmessungen die Komplement-induzierte Zelllyse sowie die
Effekte von IVIG untersucht. Klinische Daten wurden retrospektiv ausgewertet.
Fünf Patienten/Patientinnen mit hohen Autoantikörpertitern gegen NF-155 und ein
Patient mit Anti-Pan-Neurofascin Autoantikörpern konnten in der Studie detektiert
werden. Der Patient mit Autoantikörpern gegen alle drei Isoformen des Neurofascins und
IgG3-Prädominanz zeigte die deutlichste Komplementablagerung. Bei drei
Patienten/Patientinnen, die IgG1, IgG2 und IgG4 aufwiesen, war eine Aktivierung des
Komplementsystems zu beobachten, während bei zwei Patienten mit prädominanter
IgG4-Antikörpersubklasse keine Komplementablagerung nachweisbar war. Bei
Letzteren war eine Therapie mit IVIG in der Vorgeschichte erfolglos, während es bei zwei
der Patienten/Patientinnen mit anderen IgG-Subklassen und Komplementbindung unter
IVIG Therapie zu einer mäßigen bis deutlichen Symptombesserung in der Akutphase
kam. Eine Koinkubation mit IVIG führte in den ELISA basierten und zellbasierten
Versuchen zu keinem Effekt auf die Autoantikörperbindung an das Zielantigen, jedoch
zu einer deutlichen Reduktion der Antikörper-vermittelten Komplementbindung. Diese
Reduktion war sowohl bei Koinkuabtion von IVIG mit dem Komplementfaktor C1q als
auch bei Präinkubation von IVIG vor C1q Gabe zu sehen. Bei zwei der
Patienten/Patientinnen mit hohen Komplementablagerungen konnte eine erhöhte
Zytotoxizität nachgewiesen werden, welche bei Zugabe von IVIG verringert wurde.
Schlussfolgernd ist die Autoantikörper-induzierte Komplementablagerung abhängig von
der prädominanten IgG Subklasse. IVIG führt zu einer deutlichen,
konzentrationsabhängigen Reduktion der Komplementablagerung, sowie möglicher
zytotoxischer Effektorfunktionen wie die Zytolyse myelinisierter Schwannzellen oder
Nervenaxonen. Darüber hinaus könnte die Subklassenanalyse von Erkrankten das
Therapieansprechen auf IVIG vorhersagen und sollte daher eine wichtige Rolle in der
Diagnostik der Nodo-Paranodopathie spielen. IVIG sowie andere über das
Komplementsystem wirkende Therapeutika können in der Behandlung der schwer
betroffenen Patienten/Patientinnen, insbesondere bei Anti-Pan-Neurofascin positiver
Neuropathie, in Betracht gezogen werden.
Multiple Sklerose (MS) ist eine chronisch-entzündliche Erkrankung des Zentralen Nervensystems mit deutlich ausgeprägten Autoimmunphänomenen. Das derzeit meistverwendete Therapeutikum zur Sekundärprophylaxe von Krankheitsschüben ist rekombinantes Interferon-β (IFN-β). Wirk- und Nebenwirkungsmechanismen des Medikaments werden bisher nur partiell verstanden. In der Pathogenese der MS spielt eine Familie chemotaktisch wirksamer Zytokine, der Chemokine, eine entscheidende Rolle. Ziel dieser Studie war zu untersuchen, ob IFN-β die systemischen Konzentrationen der Pathogenese-relevanten Chemokine CXCL10, CCL2 und außerdem des endogenen Pyrogens IL-6 verändert, und ob diese Veränderungen mit dem Auftreten grippeartiger Nebenwirkungen korrelieren. Zu diesem Zweck wurden bei 37 Patienten mit schubförmiger MS zu drei Zeitpunkten – vor sowie 6 und 24 Stunden nach der Applikation von IFN-β – die genannten Botenstoffe im Blut bestimmt. Parallel wurden subjektiv empfundene grippeartige Nebenwirkungen mit Hilfe eines standardisierten Fragebogens abgefragt, und die Körperkerntemperatur wurde gemessen. Als Kontrollen dienten gesunde Probanden, derzeit nicht immunmodulatorisch behandelte MS-Patienten und MS-Patienten unter Therapie mit Glatirameracetat. Nur bei den mit IFN-β behandelten Patienten zeigte sich nach 6 Stunden ein signifikanter transienter Anstieg der Konzentrationen von CXCL10, CCL2. Der Anstieg der Chemokinkonzentrationen korrelierte mit einem transienten IL-6-Anstieg und dem Auftreten grippeartiger Nebenwirkungen. Chemokine, unter denen sich zahlreiche starke endogene Pyrogene befinden, könnten somit für die häufig zu beobachtenden grippeartigen Nebenwirkungen mit verantwortlich sein. Die Ergebnisse werfen die weiterführende Frage auf, ob die beobachtete Chemokininduktion auch relevant für den therapeutischen Effekt von IFN- ist. Ob Chemokine sich erfolgreich als Biomarker zur Prädiktion des Therapieerfolgs einsetzen lassen, wird derzeit in einem weiterführenden Projekt untersucht.
GTP cyclohydrolase 1, encoded by the GCH1 gene, is an essential enzyme for dopamine production in nigrostriatal cells. Loss-of-function mutations in GCH1 result in severe reduction of dopamine synthesis in nigrostriatal cells and are the most common cause of DOPA-responsive dystonia, a rare disease that classically presents in childhood with generalized dystonia and a dramatic long-lasting response to levodopa. We describe clinical, genetic and nigrostriatal dopaminergic imaging ([(123)I]N-ω-fluoropropyl-2β-carbomethoxy-3β-(4-iodophenyl) tropane single photon computed tomography) findings of four unrelated pedigrees with DOPA-responsive dystonia in which pathogenic GCH1 variants were identified in family members with adult-onset parkinsonism. Dopamine transporter imaging was abnormal in all parkinsonian patients, indicating Parkinson's disease-like nigrostriatal dopaminergic denervation. We subsequently explored the possibility that pathogenic GCH1 variants could contribute to the risk of developing Parkinson's disease, even in the absence of a family history for DOPA-responsive dystonia. The frequency of GCH1 variants was evaluated in whole-exome sequencing data of 1318 cases with Parkinson's disease and 5935 control subjects. Combining cases and controls, we identified a total of 11 different heterozygous GCH1 variants, all at low frequency. This list includes four pathogenic variants previously associated with DOPA-responsive dystonia (Q110X, V204I, K224R and M230I) and seven of undetermined clinical relevance (Q110E, T112A, A120S, D134G, I154V, R198Q and G217V). The frequency of GCH1 variants was significantly higher (Fisher's exact test P-value 0.0001) in cases (10/1318 = 0.75%) than in controls (6/5935 = 0.1%; odds ratio 7.5; 95% confidence interval 2.4-25.3). Our results show that rare GCH1 variants are associated with an increased risk for Parkinson's disease. These findings expand the clinical and biological relevance of GTP cycloydrolase 1 deficiency, suggesting that it not only leads to biochemical striatal dopamine depletion and DOPA-responsive dystonia, but also predisposes to nigrostriatal cell loss. Further insight into GCH1-associated pathogenetic mechanisms will shed light on the role of dopamine metabolism in nigral degeneration and Parkinson's disease.
Inflammation in the brain and gut is a critical component of several neurological diseases, such as Parkinson’s disease (PD). One trigger of the immune system in PD is aggregation of the pre-synaptic protein, α-synuclein (αSyn). Understanding the mechanism of propagation of αSyn aggregates is essential to developing disease-modifying therapeutics. Using a brain-first mouse model of PD, we demonstrate αSyn trafficking from the brain to the ileum of male mice. Immunohistochemistry revealed that the ileal αSyn aggregations are contained within CD11c+ cells. Using single-cell RNA sequencing, we demonstrate that ileal CD11c\(^+\) cells are microglia-like and the same subtype of cells is activated in the brain and ileum of PD mice. Moreover, by utilizing mice expressing the photo-convertible protein, Dendra2, we show that CD11c\(^+\) cells traffic from the brain to the ileum. Together these data provide a mechanism of αSyn trafficking between the brain and gut.
The role of innate and adaptive inflammation as a primary driver or modifier of neuropathy in premorbidly normal nerves, and as a critical player in amplifying neuropathies of other known causes (e.g., genetic, metabolic) is incompletely understood and under-researched, despite unmet clinical need. Also, cellular and humoral components of the adaptive and innate immune system are substantial disease modifying agents in the context of neuropathies and, at least in some neuropathies, there is an identified tight interrelationship between both compartments of the immune system. Additionally, the quadruple relationship between Schwann cell, axon, macrophage, and endoneurial fibroblast, with their diverse membrane bound and soluble signalling systems, forms a distinct focus for investigation in nerve diseases with inflammation secondary to Schwann cell mutations and possibly others. Identification of key immunological effector pathways that amplify neuropathic features and associated clinical symptomatology including pain should lead to realistic and timely possibilities for translatable therapeutic interventions using existing immunomodulators, alongside the development of novel therapeutic targets.
Background
Fabry-associated pain may be the first symptom of Fabry disease (FD) and presents with a unique phenotype including mostly acral burning triggerable pain attacks, evoked pain, pain crises, and permanent pain. We recently developed and validated the first Fabry Pain Questionnaire (FPQ) for adult patients. Here we report on the validation of the self-administered version of the FPQ that no longer requires a face-to-face interview but can be filled in by the patients themselves allowing more flexible data collection.
Methods
At our Würzburg Fabry Center for Interdisciplinary Treatment, Germany, we have developed the self-administered version of the FPQ by adapting the questionnaire to a self-report version. To do this, consecutive Fabry patients with current or past pain history (n = 56) were first interviewed face-to-face. Two weeks later patients’ self-reported questionnaire results were collected by mail (n = 55). We validated the self-administered version of the FPQ by assessing the inter-rater reliability agreement of scores obtained by supervised administration and self-administration of the FPQ.
Results
The FPQ contains 15 questions on the different pain phenotypes, on pain development during life with and without therapy, and on impairment due to pain. Statistical analysis showed that the majority of questions were answered in high agreement in both sessions with a mean AC1-statistic of 0.857 for 55 nominal-scaled items and a mean ICC of 0.587 for 9 scores.
Conclusions
This self-administered version of the first pain questionnaire for adult Fabry patients is a useful tool to assess Fabry-associated pain without a time-consuming face-to-face interview but via a self-reporting survey allowing more flexible usage.
Der M. Fabry ist eine X-chromosomal vererbte lysosomale Speicherkankheit, die zu einem Multiorganversagen führt. Ein frühes Symptom sind Schmerzen, die meist schon in der frühen Kindheit einsetzen. Das Besondere an diesen Schmerzen ist, dass sie sich sehr unterschiedlich u.a. bezüglich ihres Verlaufs, ihrer Dauer und ihrer Lokalisation präsentieren können. Diese Fabry-assoziierten Schmerzen sind meist brennend und akral betont, können aber auch andere Qualitäten aufweisen und sehr variable Körperpartien erfassen, was ihre diagnostische Einordnung erschwert. Bisher verfügbare validierte Schmerzfragebögen können das Spektrum der Fabry-assoziierten Schmerzen nicht erfassen. In dieser Arbeit wird der erste M. Fabry spezifische Schmerzfragebogen für Erwachsene in zwei Versionen präsentiert. Die erste Version ist eine Interview Version (iFPQ), bei der der Arzt in einem persönlichen Gespräch mit dem Patienten mit Hilfe des Fragbogens alle wesentlichen Aspekte der Fabry-assoziierten Schmerzen erfragen kann. Die zweite Version kann eigenständig vom Patienten ausgefüllt werden (saFPQ). Zur Etablierung der Fragebögen wurde in einer Pilotstudie zunächst mit 20 Patienten eine erste Version des iFPQ entwickelt. Nach Verbesserungen wurde die Interview Version mit Hilfe von 42 Studienteilnehmern validiert, die jeweils an einem Erst- und Zweitgespräch im Abstand von zwei Wochen teilnahmen. Hierbei wurde auch der NPSI als vergleichender Fragebogen ausgefüllt. Bei der ersten statistischen Analyse ergab sich eine gute Reliabilität mit ICC-Werten von 0,896 bis 0,999 aber eine unzureichende Validität zwischen iFPQ und NPSI mit K-Werten von 0,257 bis 0,566. Nach der ersten statistischen Analyse wurde der Fragebogen erneut überarbeitet und mit Hilfe von 20 Studienteilnehmern erneut validiert. Anschließend zeigte sich eine gute Validität mit K-Werten von 0,634 bis 1,0. Der saFPQ wurde im Anschluss an die finale iFPQ Version entwickelt. Bei 40 Patienten erfolgte ein Erstgespräch, bei dem die Patienten die valide Version des iFPQ ausfüllten. Im Abstand von zwei Wochen schickten die Patienten dann die selbständig ausgefüllte Version des saFPQ postalisch zurück. Die postalische Version erweitert die Flexibilität dieses Fragebogens. Sie ist für den klinischen Alltag sehr relevant. Die Resonanz der Patienten hinsichtlich beider Fragebögen war sehr positiv. Perspektivisch ist die Entwicklung einer englischen Version geplant.
Autophagy-mediated degradation of synaptic components maintains synaptic homeostasis but also constitutes a mechanism of neurodegeneration. It is unclear how autophagy of synaptic vesicles and components of presynaptic active zones is regulated. Here, we show that Pleckstrin homology containing family member 5 (Plekhg5) modulates autophagy of synaptic vesicles in axon terminals of motoneurons via its function as a guanine exchange factor for Rab26, a small GTPase that specifically directs synaptic vesicles to preautophagosomal structures. Plekhg5 gene inactivation in mice results in a late-onset motoneuron disease, characterized by degeneration of axon terminals. Plekhg5-depleted cultured motoneurons show defective axon growth and impaired autophagy of synaptic vesicles, which can be rescued by constitutively active Rab26. These findings define a mechanism for regulating autophagy in neurons that specifically targets synaptic vesicles. Disruption of this mechanism may contribute to the pathophysiology of several forms of motoneuron disease.
Background
Mucopolysaccharidosis type III (Sanfilippo syndrome) is a lysosomal storage disorder, caused by a deficiency in the heparan-N-sulfatase enzyme involved in the catabolism of the glycosaminoglycan heparan sulfate. It is characterized by early nonspecific neuropsychiatric symptoms, followed by progressive neurocognitive impairment in combination with only mild somatic features. In this patient group with a broad clinical spectrum a significant genotype-phenotype correlation with some mutations leading to a slower progressive, attenuated course has been demonstrated.
Case presentation
Our patient had complications in the neonatal period and was diagnosed with Mucopolysaccharidosis IIIa only at the age of 28 years. He was compound heterozygous for the variants p.R245H and p.S298P, the latter having been shown to lead to a significantly milder phenotype.
Conclusions
The diagnostic delay is even more prolonged in this patient population with comorbidities and a slowly progressive course of the disease.
Zu den häufigsten Symptomen der Neuroborreliose zählen Meningopolyradikulitis, lymphozytäre Meningitis und eine Beteiligung von Hirnnerven. Die Diagnosestellung erfolgt anhand klinischer Symptomatik, Liquoranalyse und Antikörperuntersuchungen von Liquor und Serum. Besonders in der Frühphase der Infektion gestaltet sich die Diagnosesicherung aufgrund der noch fehlenden Antikörperreaktion jedoch oftmals sehr schwierig.
Die Ergebnisse mehrerer Studien legen nahe, dass CXCL-13 einen wertvollen Beitrag leisten könnte, um diese diagnostische Lücke zu schließen. Ziel der Studie war es, die Wertigkeit des Liquorbiomarkers CXCL-13 in der Diagnostik der Neuroborreliose und anderer neuroinflammatorischer Erkrankungen anhand eines großen unselektierten Kollektivs zu ermitteln, einen cut-off Wert für die Verwendung im klinischen Alltag zu definieren und die Ergebnisse mit der bestehenden Literatur zu vergleichen.
Dafür wurden am Klinikum Wels-Grieskirchen über einen Zeitraum von 3 Jahren alle Patienten analysiert, bei denen im Rahmen einer Liquorpunktion die CXCL-13 Konzentration bestimmt wurde. Die Patienten wurden anhand der Hauptdiagnose bei Entlassung in 12 Diagnosegruppen aufgeteilt, für die Einteilung der Neuroborreliose Patienten wurden die Diagnoseleitlinien der DGN herangezogen.
Alle Liquorproben wurden routinemäßig auf Zellzahl, Gesamteiweiß und Laktat untersucht, die CXCL-13 Konzentration wurde anhand eines enzyme-linked immunsorbent assay (CXCL-13 ELISA, Euroimmun) bestimmt.
Unter den 1410 augewerteten Patienten fanden sich 29 Fälle mit gesicherter Neuroborreliose sowie 9 Fälle mit wahrscheinlicher/möglicher Neuroborreliose. Beide Neuroborreliosegruppen zeigten eine deutlich erhöhte mediane CXCL-13 Konzentration (554pg/ml bzw. 649pg/ml), in der Gruppe der bakteriellen und Pilzinfektionen (n=6) fand sich ebenfalls ein deutlich erhöhter Median von 410pg/ml. Alle anderen Gruppen wiesen signifikant niedrigere CXCL-13 Konzentrationen auf (p<0,001), lediglich bei sechs Patienten aus der Gruppe der soliden Tumore, darunter ein kutanes Lymphom und fünf hirneigene Tumore, wurden Werte über 500pg/ml gefunden.
Anhand einer ROC-Kurve wurde der ideale cut-off für die Diagnose der gesicherten Neuroborreliose errechnet. Dieser lag bei 55,5pg/ml mit einer Sensitivität von 96,6% (95% KI 80,4-99,8%) und einer Spezifität von 94,9% (95% KI 93,5-95,9%).
Bei 28 der 29 gesicherten Neuroborreliosefälle konnte ein positiver Antikörperindex nachgewiesen werden, dies entspricht einer Sensitivität von 96,6%. Der direkte Erregernachweis mittels PCR wurde bei neun Patienten durchgeführt, er war lediglich in zwei Fällen positiv, die Sensitivität lag bei 22,2%.
Bei den Patienten mit wahrscheinlicher Neuroborreliose (n=5) war eine Bestimmung des Antikörper-Index nicht möglich, da entweder nur im Serum oder im Liquor borrelienspezifische Antikörper vorlagen. Alle Patienten zeigten eine typische klinische Symptomatik, eine lymphozytäre Pleozytose und deutlich erhöhte CXCL-13 Konzentrationen. Es erfolgte eine antibiotische Therapie mit Ceftriaxon, worauf die Symptomatik rasch rückläufig war.
Die Ergebnisse der Studie bestätigen die hohe Wertigkeit von CXCL-13 für die Diagnose der Neuroborreliose und belegen die Übertragbarkeit der bisherigen Funde auf ein unselektiertes Patientenkollektiv. Die CXCL-13 Bestimmung ist dem direkten Erregernachweis deutlich überlegen, dieser ist aufgrund der niedrigen Sensitivität lediglich als Bestätigungstest geeignet. Im Vergleich zum borrelienspezifischen Antikörper-Index ist CXCL-13 als in etwa ebenbürtig anzusehen. Besonders in der Frühphase der Infektion bietet die CXCL-13 Bestimmung aufgrund der Latenzzeit bis zur Nachweisbarkeit von Antikörpern jedoch einen deutlichen Vorteil. Zusätzlich fällt die CXCL-13 Konzentration nach erfolgter Therapie rasch wieder ab, wodurch es sich auch als Verlaufsparameter eignet. Ein positiver Antikörper-Index kann über viele Jahre persistieren, weshalb eine Unterscheidung zwischen akuter und abgelaufener Infektion unmöglich ist. Am sinnvollsten erscheint eine Kombination von CXCL-13 und Antikörper-Index, in Verbindung mit der klinischen Präsentation ergibt sich so eine sehr hohe diagnostische Sicherheit in allen Stadien der Erkrankung.
Background:
Cerebellar liponeurocytoma is an extremely rare tumour entity of the central nervous system. It is histologically characterised by prominent neuronal/neurocytic differentiation with focal lipidisation and corresponding histologically to WHO grade II. It typically develops in adults, and usually shows a low proliferative potential. Recurrences have been reported in almost 50% of cases, and in some cases the recurrent tumour may display increased mitotic activity and proliferation index, vascular proliferations and necrosis. Thus pathological diagnosis of liponeurocytoma is challenging. This case presentation highlights the main clinical, radiographic and pathological features of a cerebellar liponeurocytoma.
Case presentation:
A 59-year-old, right-handed woman presented at our department with a short history of persistent headache, vertigo and gait disturbances. Examination at presentation revealed that the patient was awake, alert and fully oriented. The cranial nerve status was normal. Uncertainties were noted in the bilateral finger-to-nose testing with bradydiadochokinesis on both sides. Strength was full and no pronator drift was observed. Sensation was intact. No signs of pyramidal tract dysfunction were detected. Her gait appeared insecure. The patient underwent surgical resection. Afterward no further disturbances could be detected.
Conclusions:
To date >40 cases of liponeurocytoma have been reported, including cases with supratentorial location. A review of the 5 published cases of recurrent cerebellar. Liponeurocytoma revealed that the median interval between the first and second relapse was rather short, indicating uncertain malignant potential. The most recent WHO classification of brain tumours (2016) classifies the cerebellar liponeurocytoma as a separate entity and assigns the tumour to WHO grade II. Medulloblastoma is the most important differential diagnosis commonly seen in children and young adults. In contrast, cerebellar liponeurocytoma is typically diagnosed in adults. The importance of accurate diagnosis should not be underestimated especially in the view of possible further therapeutic interventions and for the determination of the patient's prognosis.
From November 2nd - 4th 2012, the 4th NEUROWIND e.V. meeting was held in Motzen, Brandenburg, Germany. Again more than 60 participants, predominantly at the doctoral student or postdoc level, gathered to share their latest findings in the fields of neurovascular research, neurodegeneration and neuroinflammation. Like in the previous years, the symposium provided an excellent platform for scientific exchange and the presentation of innovative projects in the stimulating surroundings of the Brandenburg outback. This year’s keynote lecture on the pathophysiological relevance of neuronal networks was given by Christian Gerloff, Head of the Department of Neurology at the University Clinic of Hamburg-Eppendorf. Another highlight of the meeting was the awarding of the NEUROWIND e.V. prize for young scientists working in the field of experimental neurology. The award is donated by the Merck Serono GmbH, Darmstadt, Germany and is endowed with 20.000 Euro. This year the jury decided unanimously to adjudge the award to Michael Gliem from the Department of Neurology at the University Clinic of Düsseldorf (group of Sebastian Jander), Germany, for his outstanding work on different macrophage subsets in the pathogenesis of ischemic stroke published in the Annals of Neurology in 2012.
From october 25th - 27th 2013, the 5th NEUROWIND e.V. meeting was held in Motzen, Brandenburg, Germany. This year more than 60 doctoral students and postdocs from over 25 different groups working in German university hospitals or research institutes attended the meeting to discuss their latest findings in the fields of neuroimmunology, neurodegeneration and neurovascular research. All participants appreciated the stimulating environment in Motzen, Brandenburg, and people took the opportunity for scientific exchange, discussion about ongoing projects and already started further collaborations. Like in the previous years, the symposium was regarded as a very well organized platform to support research of young investigators in Germany.
According to the major aim of NEUROWIND e.V. to support younger researchers in Germany the 3rd NEUROWIND YOUNG SCIENTIST AWARD for experimental neurology was awarded to Ruth Stassart working in the group of Klaus Armin Nave and Wolfgang Brück (MPI Göttingen and Department of Neuropathology, Göttingen Germany). The successful work was published in Nature Neuroscience entitled “A role for Swann cell-derived neuregulin-1 in remyelination”. This outstanding paper deals with the function of Schwann cell neuregulin as an endogenous factor for myelin repair. The award is endowed with 20.000 Euro sponsored by Merck Serono GmbH, Darmstadt, Germany (unrestricted educational grant). This year’s keynote lecture was given by Albert Ludolph, Head of the Department of Neurology at the University Clinic of Ulm. Dr. Ludolph highlighted the particular role of individual scientists for the development of research concepts in Alzheimer´s disease (AD) and frontotemporal dementia (FTD).
Background
The aim of the present study was to assess manifestations of and applied treatment concepts for females with Fabry disease (FD) according to the current European Fabry Guidelines.
Methods
Between 10/2008 and 12/2014, data from the most recent visit of 261 adult female FD patients from six German Fabry centers were retrospectively analyzed. Clinical presentation and laboratory data, including plasma lyso-Gb3 levels were assessed.
Results
Fifty-five percent of females were on enzyme replacement therapy (ERT), according to recent European FD guidelines. Thirty-three percent of females were untreated although criteria for ERT initiation were fulfilled. In general, the presence of left ventricular hypertrophy (LVH) seemed to impact more on ERT initiation than impaired renal function. In ERT-naïve females RAAS blockers were more often prescribed if LVH was present rather than albuminuria. Affected females with missense mutations showed a similar disease burden compared to females with nonsense mutations. Elevated plasma lyso-Gb3 levels in ERT-naïve females seem to be a marker of disease burden, since patients showed comparable incidences of organ manifestations even if they were ~8 years younger than females with normal lyso-Gb3 levels.
Conclusion
The treatment of the majority of females with FD in Germany is in line with the current European FD guidelines. However, a relevant number of females remain untreated despite organ involvement, necessitating a careful reevaluation of these females.
Beim ischämischen Schlaganfall führt ein thrombotischer Verschluss von gehirnversorgenden Arterien zu einer akuten Durchblutungsstörung, mit der Folge von neurologischen Defiziten. Primäres Therapieziel ist es, diese Blutgerinnsel aufzulösen, um die Sauerstoffversorgung des Gehirns wiederherzustellen und den ischämischen Hirnschaden zu begrenzen. Dazu stehen die intravenösen Thrombolyse mit rt-PA (rekombinanter Gewebe-Plasminogen-Aktivator) sowie die endovaskuläre mechanische Thrombektomie zur Verfügung. Häufig kann ein Schlaganfall, trotz erfolgreicher Rekanalisation der Gefäße, zu einer weiteren Größenzunahme des Infarktes und neurologischen Defiziten bei den Patienten führen. Diese Größenzunahme beruht zum einen auf einem sich entwickelnden Hirnödem und zum anderen auf entzündlichen Prozessen. Zahlreiche Hinweise deuten darauf hin, dass der Schlaganfall ein Zusammenspiel aus thrombotischen und entzündlichen Ereignissen ist, ein Phänomen, das als Thromboinflammation bezeichnet wird. Aufgrund der begrenzten Behandlungsmöglichkeiten ist die Entwicklung neuer Therapieansätze für den ischämischen Schlaganfall besonders wichtig. Agaphelin und Ixolaris sind Proteine aus den Speicheldrüsen von Hämatophagen, für welche in früheren Studien eine starke antithrombotische Wirkung bei gleichzeitig geringem Blutungsrisiko nachgewiesen wurde. Diese möglichen antithrombotischen Effekte wurden in dieser Studie im Hinblick auf ihre Wirksamkeit und Sicherheit im Mausmodell der zerebralen Ischämie untersucht. Die Behandlung der Mäuse mit Agaphelin 1 Stunde nach transienter Okklusion der Arteria cerebri media (tMCAO) führte zu kleineren Schlaganfallvolumina und geringeren neurologischen Defiziten an Tag 1 nach dem Schlaganfall. Die Mortalität der Mäuse war bis Tag 7 deutlich gesunken. Aus klinischer Sicht ist ebenfalls relevant, dass der starke antithrombotische Effekt von Agaphelin im Mausmodell nicht mit einem erhöhten Risiko für intrazerebrale Blutungen einherging. Diesem protektiven Effekt von Agaphelin lagen eine verminderte intrazerebrale Thrombusbildung, eine abgeschwächte Entzündungsantwort und eine Stabilisierung der Blut-Hirn-Schranke sowie eine Reduzierung der Apoptose zugrunde. Nach der Gabe von Ixolaris 1 Stunde nach tMCAO waren zwar signifikant geringere Infarktgrößen messbar, diese führten allerdings nicht zu einer Verbesserung der neurologischen Defizite. Zudem verursachte die Gabe von Ixolaris schon 24 Stunden nach tMCAO erhebliche intrazerebrale Blutungen und auch die Mortalität der Mäuse war zu diesem Zeitpunkt bereits erhöht. Aufgrund dieser massiven Nebenwirkungen scheint Ixolaris kein geeigneter Kandidat für eine humane Anwendung zu sein. Bei Agaphelin hingegen könnte es
sich um einen vielversprechenden Kandidaten für die Behandlung des ischämischen Schlaganfalls handeln. Vor einer möglichen Testung von Agaphelin in klinischen Studien, sind weitere translationale Untersuchungen notwendig, um ein noch präziseres Verständnis für die Wirksamkeit und Sicherheit von Agaphelin zu gewinnen. Insgesamt stellt die Hemmung thromboinflammatorischer Prozesse, ohne eine Erhöhung der Blutungskomplikationen, eine vielversprechende Option zur Behandlung des ischämischen Schlaganfalls dar.
In dieser Dissertation untersuchten wir die neuronalen Korrelate des Training-Effektes einer auditorischen P300 Gehirn-Computer Schnittstelle mittels fMRI Analyse in einem prä-post Design mit zehn gesunden Testpersonen. Wir wiesen in drei Trainings-sitzungen einen Trainingseffekt in der EEG-Analyse der P300 Welle nach und fanden entsprechende Kontraste in einer prä-post Analyse von fMRI Daten, wobei in allen fünf Sitzungen das gleiche Paradigma verwendet wurde. In der fMRI Analyse fanden wir fol-gende Ergebnisse: in einem Target-/ Nichttarget Kontrast zeigte sich verstärkte Aktivie-rung in Generatorregionen der P300 Welle (temporale und inferiore frontale Regionen) und interessanterweise auch in motorassoziierten Arealen, was höhere kognitiver Pro-zesse wie Aufmerksamkeitslenkung und Arbeitsspeicher widerspiegeln könnte. Der Kon-trast des Trainingseffektes zeigte nach dem Training einen stärkeren Rebound Effekt im Sinne einer verstärkten Aktivierung in Generatorregionen der P300 Welle, was eine ver-besserte Erkennung und Prozessierung von Target-Stimuli reflektieren könnte. Eine Ab-nahme von Aktivierung in frontalen Arealen in diesem Kontrast könnte durch effizientere Abläufe kognitiver Prozesse und des Arbeitsgedächtnis erklärt werden.
microRNAs in chronic pain

Chronic pain is a common problem in clinical practice, not well understood clinically, and frequently tough to satisfactorily diagnose. Because the pathophysiology is so complex, finding effective treatments for people with chronic pain has been overall less than successful and typically reduced to an unsatisfactory trial-and-error process, all of which translates into a significant burden to society. Knowledge of the mechanisms underlying the development of chronic pain, and moreover why some patients experience pain and others not, may aid in developing specific treatment regimens. Although nerve injuries are major contributors to pain chronification, they cannot explain the entire phenomenon. Considerable research has underscored the importance of the immune system for the development and maintenance of chronic pain, albeit the exact factors regulating inflammatory reactions remain unclear. Understanding the putative molecular and cellular regulator switches of inflammatory reactions will open novel opportunities for immune modulatory analgesics with putatively higher specificity and less adverse effects. It has become clear that small, non- coding RNA molecules known as microRNAs are in fact potent regulators of many thousands of genes and possibly cross-communicate between cellular pathways in multiple systems acting as so-called “master-switches”. Aberrant expression of miRNAs is now implicated in numerous disorders, including nerve injuries as well as in inflammatory processes. Moreover, compelling evidence supports the idea that miRNAs also regulate pain, and in analogy to the oncology field aid in the differential diagnosis of disease subtypes. In fact, first reports describing characteristic miRNA expression profiles in blood or cerebrospinal fluid of patients with distinct pain conditions are starting to emerge, however evidence linking specific miRNA expression profiles to specific pain disorders is still insufficient. The present thesis aimed at first, identifying specific miRNA signatures in two distinct chronic pain conditions, namely peripheral neuropathies of different etiologies and fibromyalgia syndrome. Second, it aimed at identifying miRNA profiles to better understand potential factors that differentiate painful from painless neuropathies and third, study the mechanistic role of miRNAs in the pathophysiology of pain, to pave the way for new druggable targets.
Three studies were conducted in order to identify miRNA expression signatures that are characteristic for the given chronic pain disorder. The first study measured expression of miR-21, miR-146a and miR-155 in white blood cells, skin and nerve biopsies of patients with peripheral neuropathies. It shows that peripheral neuropathies of different etiologies are associated with increased peripheral miR-21 and miR-146a, but decreased miR-155 expression. More importantly, it was shown that painful neuropathies have increased sural nerve miR-21 and miR-155 expression, but reduced miR-146a and miR-155 expression in distal skin of painful neuropathies. These results point towards the potential use of miRNAs profiles to stratify painful neuropathies. The seconds study extends these findings and first analyzed the role of miR-132-3p in patients and subsequently in an animal model of neuropathic pain. Interestingly, miR-132-3p was upregulated in white blood cells and sural nerve biopsies of patients with painful neuropathies and in animals after spared nerve injury. Pharmacologically modulating the expression of miR-132-3p dose-dependently reversed pain behavior and pain aversion, indicating the pro-nociceptive effect of miR-132-3p in chronic pain. This study thus demonstrates the potential analgesic impact by modulating miRNA expression. Fibromyalgia is associated with chronic widespread pain and, at least in a subgroup, impairment in small nerve fiber morphology and function. Interestingly, the disease probably comprises subgroups with different underlying pathomechanisms. In accordance with this notion, the third study shows that fibromyalgia is associated with both aberrant white blood cell and cutaneous miRNA expression. Being the first of its kind, this study identified miR-let-7d and its downstream target IGF-1R as potential culprit for impaired small nerve fiber homeostasis in a subset of patients with decreased intra-epidermal nerve fiber density. The work presented in this thesis is a substantial contribution towards the goal of better characterizing chronic pain based on miRNA expression signatures and thus pave the way for new druggable targets.
Treating seronegative neuromyelitis optica spectrum disorder with inebilizumab: a case report

Background
Neuromyelitis optica spectrum disorder (NMOSD) is a devastating inflammatory disease of the central nervous system that is often severely disabling from the outset. The lack of pathognomonic aquaporin 4 (AQP4) antibodies in seronegative NMOSD not only hinders early diagnosis, but also limits therapeutic options, in contrast to AQP4 antibody-positive NMOSD, where the therapeutic landscape has recently evolved massively.
Case presentation
We report a 56-year-old woman with bilateral optic neuritis and longitudinally extensive myelitis as the index events of a seronegative NMOSD, who was successfully treated with inebilizumab.
Conclusion
Treatment with inebilizumab may be considered in aggressive seronegative NMOSD. Whether broader CD19-directed B cell depletion is more effective than treatment with rituximab remains elusive.
In der vorliegenden Arbeit wurde mittels der Whole-Cell Patch-Clamp Methode sensible Neurone von transgenen Mäusen untersucht, bei denen das Gen für TRPV1 (transient receptor potential V1) deletiert wurde. Das Ergebniss wurde mit den Daten von Wildtyp Mäusen verglichen. TRPV1 (früher VR1; vanilloid receptor 1) wird nahezu selektiv in sensiblen Neuronen exprimiert und wird im heterologen Expressionssystem durch Vanilloide, Hitze (> 43°C) und Protonen aktiviert. Durch diese Eigenschaften scheint TRPV1 für die rezeptiven Eigenschaften polymodaler Nozizeptoren von großer Bedeutung zu sein. Als ein Model des peripheren afferenten Neurons wurde die Aktivierbarkeit kultivierter Spinalganglienzellen durch Vanilloide, Protonen und Hitze elektrophysiologisch untersucht. Während etwa 35% der Wildtyp-Zellen Vanilloid-sensibel waren, fehlte in Zellen der TRPV1-knockout Maus jegliche Vanilloid-Sensibilität. Auch bei der Protonen-Sensibilität wurde eine signifikante Reduktion in TRPV1-knockout Zellen beobachtet. In Wildtyp-Zellen wurde eine hohe Protonen-Sensibilität fast ausschliesslich in Vanilloid-sensiblen Zellen beobachtet. Hitze-induzierte Einwärtsströme mit einer Aktivierungsschwelle bei 43°C wurden ausschliesslich in Vanilloid-sensiblen Zellen der Wildtyp-Maus beobachtet. Dagegen wurden Hitze-induzierte Einwärtsströme mit einer Aktivierungsschwelle über 53°C in sowohl Wildtyp- als auch in TRPV1-knockout Zellen beobachtet. Im Bezug auf die Bedetung von TRPV1, wurde die Funktionalität zwei distinkter Populationen von Spinalganglienzellen, NGF- bzw. GDNF-abhängigen Neuronen, durch eine Lebendfärbung mit IB4-FITC untersucht. Hinsichtlich Vanilloid-, Protonen-, Hitze-Sensibilitöt wurden jedoch keine Unterschiede zwischen IB4-negative und IB4-positive Neuronen beobachtet. Die vorliegende Studie zeigt damit, dass TRPV1 für Vanilliod-Sensibilität sensibler Neurone essentiell ist. Weiterhin konnte gezeigt werden, dass TRPV1 ein wichtiges Transduktionselement für sowohl die Protonen-Sensibilität als auch für die Hitze-Sensibilität in Spinalganglienzellen darstellt. Die Daten dieser zellulären Untersuchungen konnten in weiteren in vitro und in vivo Untersuchungen bestätigt werden (Caterina et al., 2000).
Background
Anderson–Fabry disease (FD) is an X-linked lysosomal storage disorder with varying organ involvement and symptoms, depending on the underlying mutation in the alpha-galactosidase A gene (HGNC: GLA). With genetic testing becoming more readily available, it is crucial to precisely evaluate pathogenicity of each genetic variant, in order to determine whether there is or might be not a need for FD-specific therapy in affected patients and relatives at the time point of presentation or in the future.
Methods
This case series investigates the clinical impact of the specific GLA gene variant c.376A>G (p.Ser126Gly) in five (one heterozygous and one homozygous female, three males) individuals from different families, who visited our center between 2009 and 2021. Comprehensive neurological, nephrological and cardiac examinations were performed in all cases. One patient received a follow-up examination after 12 years.
Results
Index events leading to suspicion of FD were mainly unspecific neurological symptoms. However, FD-specific biomarkers, imaging examinations (i.e., brain MRI, heart MRI), and tissue-specific diagnostics, including kidney and skin biopsies, did not reveal evidence for FD-specific symptoms or organ involvement but showed normal results in all cases. This includes findings from 12-year follow-up in one patient with renal biopsy.
Conclusion
These findings suggest that p.Ser126Gly represents a benign GLA gene variant which per se does not cause FD. Precise clinical evaluation in individuals diagnosed with genetic variations of unknown significance should be performed to distinguish common symptoms broadly prevalent in the general population from those secondary to FD.
Die Pathophysiologie der PNP wie auch die Entstehung der oft assoziierten neuropathischen Schmerzen ist unklar. Gleichzeitig gibt es bislang keine geeigneten Biomarker, die die oft komplizierte Differentialdiagnose vereinfachen können. Einige Tiermodelle und klinische Studien lieferten bereits Hinweise auf die entscheidende Rolle pro- und anti-inflammatorischer Zytokine in diesen Prozessen. Ziel unserer Studie war es, die systemische Genexpression pro- und anti-inflammatorischer Zytokine in einer großen Kohorte von Patienten mit PNP verschiedener Ätiologie zu charakterisieren. Insgesamt konnten 111 PNP-Patienten und 38 gesunde Kontrollpersonen prospektiv rekrutiert werden. Nach Isolation von PBMC aus Blutproben von 97 Patienten wurde die Genexpression der pro-inflammatorischen Zytokine TNF, IL1, IL2, IL6, IL8 und der anti-inflammatorischen Zytokine IL4 und IL10 mittels qRT-PCR bestimmt. Bei 47 Patienten und 12 Kontrollen wurde zudem die IL6-, IL-8- und TNF-Zytokinproduktion von PBMC in vitro nach Stimulation durch LPS mittels ELISA untersucht. Hauptbefund war ein pro-inflammatorisches Zytokinprofil der PNP-Patienten mit höherer Genexpression von IL1, IL2, IL8 und TNF im Vergleich zu den gesunden Kontrollen. Im Falle der entzündlichen Neuropathien konnte zudem eine niedrigere Genexpression von IL10 im Vergleich zu Gesunden nachgewiesen werden. Sowohl schmerzhafte als auch schmerzlose Verlaufsformen wiesen ein pro-inflammatorisches Zytokingenexpressionsprofil im Vergleich zu Gesunden auf, das bei schmerzhaften PNP deutlich mehr beteiligte pro-inflammatorische Zytokine umfasste; relevante Unterschiede zwischen den PNP-Patienten mit und ohne Schmerz sowie der diagnostischen Subgruppen fanden sich nicht. Eine niedrigere Stimulationsschwelle der PBMC lag bei PNP-Patienten im Vergleich zu Gesunden nicht vor. Insgesamt erscheint die Rolle einzelner Zytokine als systemische Biomarker für die Differenzierung verschiedener PNP-Formen bzw. bezüglich neuropathischen Schmerzes aufgrund einer niedrigen Spezifität deutlich eingeschränkt. Dennoch sprechen unsere Ergebnisse für eine mögliche Rolle eines pro-inflammatorischen Milieus bei der Entstehung bzw. des Verlaufes verschiedener entzündlicher und nicht-entzündlicher Neuropathien und neuropathischen Schmerzes.
Background
A new thromboembolic animal model showed beneficial effects of t-PA with an infarct volume reduction of 36.8% in swiss mice. Because knock-out animal experiments for stroke frequently used C57BL76 mice we evaluated t-PA effects in this mouse strain and measured infarct volume and vascular recanalisation in-vivo by using high-field 9.4 T MRI and a 1H surface cryo coil.
Methods
Clot formation was triggered by microinjection of murine thrombin into the right middle cerebral artery (MCA). Animals (n = 28) were treated with 10 mg/kg, 5 mg/kg or no tissue plasminogen activator (t-PA) 40 min after MCA occlusion. For MR-imaging a Bruker 9.4 T animal system with a 1H surface cryo probe was used and a T2-weighted RARE sequence, a diffusion weighted multishot EPI sequence and a 3D flow-compensated gradient echo TOF angiography were performed.
Results
The infarct volume in animals treated with t-PA was significantly reduced (0.67 ± 1.38 mm3 for 10 mg/kg and 10.9 ± 8.79 mm3 for 5 mg/kg vs. 19.76 ± 2.72 mm3 ; p < 0.001) compared to untreated mice. An additional group was reperfused with t-PA inside the MRI. Already ten minutes after beginning of t-PA treatment, reperfusion flow was re-established in the right MCA. However, signal intensity was lower than in the contralateral MCA. This reduction in cerebral blood flow was attenuated during the first 60 minutes after reperfusion. 24 h after MCA occlusion and reperfusion, no difference in signal intensity of the contralateral and ipsilateral MCAs was observed.
Conclusions
We confirm a t-Pa effect using this stroke model in the C57BL76 mouse strain and demonstrate a chronological sequence MRI imaging after t-PA using a 1H surface cryo coil in a 9.4 T MRI. This setting will allow testing of new thrombolytic strategies for stroke treatment in-vivo in C57BL76 knock-out mice.
Network medicine utilizes common genetic origins, markers and co-morbidities to uncover mechanistic links between diseases. These links can be summarized in the diseasome, a comprehensive network of disease–disease relationships and clusters. The diseasome has been influential during the past decade, although most of its links are not followed up experimentally. Here, we investigate a high prevalence unmet medical need cluster of disease phenotypes linked to cyclic GMP. Hitherto, the central cGMP-forming enzyme, soluble guanylate cyclase (sGC), has been targeted pharmacologically exclusively for smooth muscle modulation in cardiology and pulmonology. Here, we examine the disease associations of sGC in a non-hypothesis based manner in order to identify possibly previously unrecognized clinical indications. Surprisingly, we find that sGC, is closest linked to neurological disorders, an application that has so far not been explored clinically. Indeed, when investigating the neurological indication of this cluster with the highest unmet medical need, ischemic stroke, pre-clinically we find that sGC activity is virtually absent post-stroke. Conversely, a heme-free form of sGC, apo-sGC, was now the predominant isoform suggesting it may be a mechanism-based target in stroke. Indeed, this repurposing hypothesis could be validated experimentally in vivo as specific activators of apo-sGC were directly neuroprotective, reduced infarct size and increased survival. Thus, common mechanism clusters of the diseasome allow direct drug repurposing across previously unrelated disease phenotypes redefining them in a mechanism-based manner. Specifically, our example of repurposing apo-sGC activators for ischemic stroke should be urgently validated clinically as a possible first-in-class neuroprotective therapy.
