Adjuvant platinum-based chemotherapy in radically resected adrenocortical carcinoma: a cohort study

Background
After radical resection, patients with adrenocortical carcinoma (ACC) frequently experience recurrence and, therefore, effective adjuvant treatment is urgently needed. The aim of the study was to investigate the role of adjuvant platinum-based therapy.
Methods
In this retrospective multicentre cohort study, we identified patients treated with adjuvant platinum-based chemotherapy after radical resection and compared them with patients without adjuvant chemotherapy. Recurrence-free and overall survival (RFS/OS) were investigated in a matched group analysis and by applying a propensity score matching using the full control cohort (n = 268). For both approaches, we accounted for immortal time bias.
Results
Of the 31 patients in the platinum cohort (R0 n = 25, RX n = 4, R1 n = 2; ENSAT Stage II n = 11, III n = 16, IV n = 4, median Ki67 30%, mitotane n = 28), 14 experienced recurrence compared to 29 of 31 matched controls (median RFS after the landmark at 3 months 17.3 vs. 7.3 months; adjusted HR 0.19 (95% CI 0.09-0.42; P < 0.001). Using propensity score matching, the HR for RFS was 0.45 (0.29-0.89, P = 0.021) and for OS 0.25 (0.09-0.69; P = 0.007).
Conclusions
Our study provides the first evidence that adjuvant platinum-based chemotherapy may be associated with prolonged recurrence-free and overall survival in patients with ACC and a very high risk for recurrence.
Arrhythmogenic cardiomyopathy (ACM) is characterized by fibro-fatty replacement of the myocardium, heart failure and life-threatening ventricular arrhythmias. Causal mutations were identified in genes encoding for proteins of the desmosomes, predominantly plakophilin-2 (PKP2) and desmoglein-2 (DSG2). We generated gene-edited knock-out iPSC lines for PKP2 (JMUi001-A-2) and DSG2 (JMUi001-A-3) using the CRISPR/Cas9 system in a healthy control iPSC background (JMUi001A). Stem cell-like morphology, robust expression of pluripotency markers, embryoid body formation and normal karyotypes confirmed the generation of high quality iPSCs to provide a novel isogenic human in vitro model system mimicking ACM when differentiated into cardiomyocytes.
Sensitization to the adipokine leptin is a promising therapeutic strategy against obesity and its comorbidities and has been proposed to contribute to the lasting metabolic benefits of Roux-en-Y gastric bypass (RYGB) surgery. We formally tested this idea using Zucker fatty fa/fa rats as an established genetic model of obesity, glucose intolerance, and fatty liver due to leptin receptor deficiency. We show that the changes in body weight in these rats following RYGB largely overlaps with that of diet-induced obese Wistar rats with intact leptin receptors. Further, food intake and oral glucose tolerance were normalized in RYGB-treated Zucker fatty fa/fa rats to the levels of lean Zucker fatty fa/+ controls, in association with increased glucagon-like peptide 1 (GLP-1) and insulin release. In contrast, while fatty liver was also normalized in RYGB-treated Zucker fatty fa/fa rats, their circulating levels of the liver enzyme alanine aminotransferase (ALT) remained elevated at the level of obese Zucker fatty fa/fa controls. These findings suggest that the leptin system is not required for the normalization of energy and glucose homeostasis associated with RYGB, but that its potential contribution to the improvements in liver health postoperatively merits further investigation.
No studies have carried out an extensive analysis of the possible association between non-syndromic pheochromocytomas and paragangliomas (PPGLs) and other malignancies. To assess >the risk of additional malignancy in PPGL, we retrospectively evaluated 741 patients with PPGLs followed-up in twelve referral centers in Italy. Incidence of second malignant tumors was compared between this cohort and Italian patients with two subsequent malignancies. Among our patients, 95 (12.8%) developed a second malignant tumor, which were mainly prostate, colorectal and lung/bronchial cancers in males, breast cancer, differentiated thyroid cancer and melanoma in females. The standardized incidence ratio was 9.59 (95% CI 5.46–15.71) in males and 13.21 (95% CI 7.52–21.63) in females. At multivariable analysis, the risk of developing a second malignant tumor increased with age at diagnosis (HR 2.50, 95% CI 1.15–5.44, p = 0.021 for 50–59 vs. <50-year category; HR 3.46, 95% CI 1.67–7.15, p < 0.001 for >60- vs. <50-year). In patients with available genetic evaluation, a positive genetic test was inversely associated with the risk of developing a second tumor (HR 0.25, 95% CI 0.10–0.63, p = 0.003). In conclusion, PPGLs patients have higher incidence of additional malignant tumors compared to the general population who had a first malignancy, which could have an impact on the surveillance strategy.
Introduction: Left ventricular (LV) dilatation and LV hypertrophy are acknowledged precursors of myocardial dysfunction and ultimately of heart failure, but the implications of abnormal LV geometry on myocardial function are not well-understood. Non-invasive LV myocardial work (MyW) assessment based on echocardiography-derived pressure-strain loops offers the opportunity to study detailed myocardial function in larger cohorts. We aimed to assess the relationship of LV geometry with MyW indices in general population free from heart failure.
Methods and Results: We report cross-sectional baseline data from the Characteristics and Course of Heart Failure Stages A-B and Determinants of Progression (STAAB) cohort study investigating a representative sample of the general population of Würzburg, Germany, aged 30–79 years. MyW analysis was performed in 1,926 individuals who were in sinus rhythm and free from valvular disease (49.3% female, 54 ± 12 years). In multivariable regression, higher LV volume was associated with higher global wasted work (GWW) (+0.5 mmHg% per mL/m\(^2\), p < 0.001) and lower global work efficiency (GWE) (−0.02% per mL/m\(^2\), p < 0.01), while higher LV mass was associated with higher GWW (+0.45 mmHg% per g/m\(^2\), p < 0.001) and global constructive work (GCW) (+2.05 mmHg% per g/m\(^2\), p < 0.01) and lower GWE (−0.015% per g/m\(^2\), p < 0.001). This was dominated by the blood pressure level and also observed in participants with normal LV geometry and concomitant hypertension.
Conclusion: Abnormal LV geometric profiles were associated with a higher amount of wasted work, which translated into reduced work efficiency. The pattern of a disproportionate increase in GWW with higher LV mass might be an early sign of hypertensive heart disease.
LMNA-related dilated cardiomyopathy is an inherited heart disease caused by mutations in the LMNA gene encoding for lamin A/C. The disease is characterized by left ventricular enlargement and impaired systolic function associated with conduction defects and ventricular arrhythmias. We hypothesized that LMNA-mutated patients' induced Pluripotent Stem Cell-derived cardiomyocytes (iPSC-CMs) display electrophysiological abnormalities, thus constituting a suitable tool for deciphering the arrhythmogenic mechanisms of the disease, and possibly for developing novel therapeutic modalities. iPSC-CMs were generated from two related patients (father and son) carrying the same E342K mutation in the LMNA gene. Compared to control iPSC-CMs, LMNA-mutated iPSC-CMs exhibited the following electrophysiological abnormalities: (1) decreased spontaneous action potential beat rate and decreased pacemaker current (I\(_f\)) density; (2) prolonged action potential duration and increased L-type Ca\(^{2+}\) current (I\(_{Ca,L}\)) density; (3) delayed afterdepolarizations (DADs), arrhythmias and increased beat rate variability; (4) DADs, arrhythmias and cessation of spontaneous firing in response to β-adrenergic stimulation and rapid pacing. Additionally, compared to healthy control, LMNA-mutated iPSC-CMs displayed nuclear morphological irregularities and gene expression alterations. Notably, KB-R7943, a selective inhibitor of the reverse-mode of the Na\(^+\)/Ca\(^{2+}\) exchanger, blocked the DADs in LMNA-mutated iPSC-CMs. Our findings demonstrate cellular electrophysiological mechanisms underlying the arrhythmias in LMNA-related dilated cardiomyopathy.
Purpose
Over the course of COVID-19 pandemic, evidence has accumulated that SARS-CoV-2 infections may affect multiple organs and have serious clinical sequelae, but on-site clinical examinations with non-hospitalized samples are rare. We, therefore, aimed to systematically assess the long-term health status of samples of hospitalized and non-hospitalized SARS-CoV-2 infected individuals from three regions in Germany.
Methods
The present paper describes the COVIDOM-study within the population-based cohort platform (POP) which has been established under the auspices of the NAPKON infrastructure (German National Pandemic Cohort Network) of the national Network University Medicine (NUM). Comprehensive health assessments among SARS-CoV-2 infected individuals are conducted at least 6 months after the acute infection at the study sites Kiel, Würzburg and Berlin. Potential participants were identified and contacted via the local public health authorities, irrespective of the severity of the initial infection. A harmonized examination protocol has been implemented, consisting of detailed assessments of medical history, physical examinations, and the collection of multiple biosamples (e.g., serum, plasma, saliva, urine) for future analyses. In addition, patient-reported perception of the impact of local pandemic-related measures and infection on quality-of-life are obtained.
Results
As of July 2021, in total 6813 individuals infected in 2020 have been invited into the COVIDOM-study. Of these, about 36% wished to participate and 1295 have already been examined at least once.
Conclusion
NAPKON-POP COVIDOM-study complements other Long COVID studies assessing the long-term consequences of an infection with SARS-CoV-2 by providing detailed health data of population-based samples, including individuals with various degrees of disease severity.

Therapeutic drug monitoring (TDM) is increasingly relevant for an individualized antibiotic therapy and subsequently a necessary tool to reduce multidrug-resistant pathogens, especially in light of diminishing antimicrobial capabilities. Critical illness is associated with profound pharmacokinetic and pharmacodynamic alterations, which challenge dose finding and the application of particularly hydrophilic drugs such as β-lactam antibiotics. Methods: Implementation strategy, potential benefit, and practicability of the developed standard operating procedures were retrospectively analyzed from January to December 2020. Furthermore, the efficacy of the proposed dosing target of piperacillin in critically ill patients was evaluated. Results: In total, 160 patients received piperacillin/tazobactam therapy and were subsequently included in the study. Of them, 114 patients received piperacillin/tazobactam by continuous infusion and had at least one measurement of piperacillin serum level according to the standard operating procedure. In total, 271 measurements were performed with an average level of 79.0 ± 46.0 mg/L. Seventy-one piperacillin levels exceeded 100 mg/L and six levels were lower than 22.5 mg/L. The high-level and the low-level group differed significantly in infection laboratory parameters (CRP (mg/dL) 20.18 ± 11.71 vs. 5.75 ± 5.33) and renal function [glomerular filtration rate (mL/min/1.75 m2) 40.85 ± 26.74 vs. 120.50 ± 70.48]. Conclusions: Piperacillin levels are unpredictable in critically ill patients. TDM during piperacillin/tazobactam therapy is highly recommended for all patients. Although our implementation strategy was effective, further strategies implemented into the daily clinical workflow might support the health care staff and increase the clinicians' alertness.
Physical and mental well-being during the COVID-19 pandemic is typically assessed via surveys, which might make it difficult to conduct longitudinal studies and might lead to data suffering from recall bias. Ecological momentary assessment (EMA) driven smartphone apps can help alleviate such issues, allowing for in situ recordings. Implementing such an app is not trivial, necessitates strict regulatory and legal requirements, and requires short development cycles to appropriately react to abrupt changes in the pandemic. Based on an existing app framework, we developed Corona Health, an app that serves as a platform for deploying questionnaire-based studies in combination with recordings of mobile sensors. In this paper, we present the technical details of Corona Health and provide first insights into the collected data. Through collaborative efforts from experts from public health, medicine, psychology, and computer science, we released Corona Health publicly on Google Play and the Apple App Store (in July 2020) in eight languages and attracted 7290 installations so far. Currently, five studies related to physical and mental well-being are deployed and 17,241 questionnaires have been filled out. Corona Health proves to be a viable tool for conducting research related to the COVID-19 pandemic and can serve as a blueprint for future EMA-based studies. The data we collected will substantially improve our knowledge on mental and physical health states, traits and trajectories as well as its risk and protective factors over the course of the COVID-19 pandemic and its diverse prevention measures.
Growth, ageing and atherosclerotic plaque development alter the biomechanical forces acting on the vessel wall. However, monitoring the detailed local changes in wall shear stress (WSS) at distinct sites of the murine aortic arch over time has been challenging. Here, we studied the temporal and spatial changes in flow, WSS, oscillatory shear index (OSI) and elastic properties of healthy wildtype (WT, n = 5) and atherosclerotic apolipoprotein E-deficient (Apoe\(^{−/−}\), n = 6) mice during ageing and atherosclerosis using high-resolution 4D flow magnetic resonance imaging (MRI). Spatially resolved 2D projection maps of WSS and OSI of the complete aortic arch were generated, allowing the pixel-wise statistical analysis of inter- and intragroup hemodynamic changes over time and local correlations between WSS, pulse wave velocity (PWV), plaque and vessel wall characteristics. The study revealed converse differences of local hemodynamic profiles in healthy WT and atherosclerotic Apoe\(^{−/−}\) mice, and we identified the circumferential WSS as potential marker of plaque size and composition in advanced atherosclerosis and the radial strain as a potential marker for vascular elasticity. Two-dimensional (2D) projection maps of WSS and OSI, including statistical analysis provide a powerful tool to monitor local aortic hemodynamics during ageing and atherosclerosis. The correlation of spatially resolved hemodynamics and plaque characteristics could significantly improve our understanding of the impact of hemodynamics on atherosclerosis, which may be key to understand plaque progression towards vulnerability.
Despite TAVR emerging as the gold standard for a broad spectrum of patients, it is associated with serious complications. In this report we present a case, where a TAVR procedure led to a perforation at the aortomitral continuity, discuss the risk factors for the occurrence of perforations and how we decided to treat the patient.
Adrenocortical carcinoma (ACC) is a rare endocrine malignancy and treatment of advanced disease is challenging. Clinical trials with multi-tyrosine kinase inhibitors in the past have yielded disappointing results. Here, we investigated fibroblast growth factor (FGF) receptors and their pathways in adrenocortical tumors as potential treatment targets. We performed real-time RT-PCR of 93 FGF pathway related genes in a cohort of 39 fresh frozen benign and malignant adrenocortical, 9 non-adrenal tissues and 4 cell lines. The expression of FGF receptors was validated in 166 formalin-fixed paraffin embedded (FFPE) tissues using RNA in situ hybridization (RNAscope) and correlated with clinical data. In malignant compared to benign adrenal tumors, we found significant differences in the expression of 16/94 FGF receptor pathway related genes. Genes involved in tissue differentiation and metastatic spread through epithelial to mesechymal transition were most strongly altered. The therapeutically targetable FGF receptors 1 and 4 were upregulated 4.6- and 6-fold, respectively, in malignant compared to benign adrenocortical tumors, which was confirmed by RNAscope in FFPE samples. High expression of FGFR1 and 4 was significantly associated with worse patient prognosis in univariate analysis. After multivariate adjustment for the known prognostic factors Ki-67 and ENSAT tumor stage, FGFR1 remained significantly associated with recurrence-free survival (HR=6.10, 95%CI: 1.78 – 20.86, p=0.004) and FGFR4 with overall survival (HR=3.23, 95%CI: 1.52 – 6.88, p=0.002). Collectively, our study supports a role of FGF pathways in malignant adrenocortical tumors. Quantification of FGF receptors may enable a stratification of ACC for the use of FGFR inhibitors in future clinical trials.
Background
Combination therapies of anorectic gut hormones partially mimic the beneficial effects of bariatric surgery. Thus far, the effects of a combined chronic systemic administration of Glucagon-like peptide-1 (GLP-1) and peptide tyrosine tyrosine 3-36 (PYY\(_{3-36}\)) have not been directly compared to Roux-en-Y gastric bypass (RYGB) in a standardized experimental setting.
Methods
High-fat diet (HFD)-induced obese male Wistar rats were randomized into six treatment groups: (1) RYGB, (2) sham-operation (shams), (3) liraglutide, (4) PYY\(_{3-36}\), (5) PYY\(_{3-36}\)+liraglutide (6), saline. Animals were kept on a free choice high- and low-fat diet. Food intake, preference, and body weight were measured daily for 4 weeks. Open field (OP) and elevated plus maze (EPM) tests were performed.
Results
RYGB reduced food intake and achieved sustained weight loss. Combined PYY\(_{3-36}\)+liraglutide treatment led to similar and plateaued weight loss compared to RYGB. Combined PYY\(_{3-36}\)+liraglutide treatment was superior to PYY\(_{3-36}\) (p ≤ 0.0001) and liraglutide (p ≤ 0.05 or p ≤ 0.01) mono-therapy. PYY\(_{3-36}\)+liraglutide treatment and RYGB also reduced overall food intake and (less pronounced) high-fat preference compared to controls. The animals showed no signs of abnormal behavior in OF or EPM.
Conclusions
Liraglutide and PYY\(_{3-36}\) combination therapy vastly mimics reduced food intake, food choice and weight reducing benefits of RYGB.
Background
Presence of clonal hematopoiesis of indeterminate potential (CHIP) is associated with a higher risk of atherosclerotic cardiovascular disease, cancer, and mortality. The relationship between a healthy lifestyle and CHIP is unknown.
Methods and Results
This analysis included 8709 postmenopausal women (mean age, 66.5 years) enrolled in the WHI (Women's Health Initiative), free of cancer or cardiovascular disease, with deep‐coverage whole genome sequencing data available. Information on lifestyle factors (body mass index, smoking, physical activity, and diet quality) was obtained, and a healthy lifestyle score was created on the basis of healthy criteria met (0 point [least healthy] to 4 points [most healthy]). CHIP was derived on the basis of a prespecified list of leukemogenic driver mutations. The prevalence of CHIP was 8.6%. A higher healthy lifestyle score was not associated with CHIP (multivariable‐adjusted odds ratio [OR] [95% CI], 0.99 [0.80–1.23] and 1.13 [0.93–1.37]) for the upper (3 or 4 points) and middle category (2 points), respectively, versus referent (0 or 1 point). Across score components, a normal and overweight body mass index compared with obese was significantly associated with a lower odds for CHIP (OR, 0.71 [95% CI, 0.57–0.88] and 0.83 [95% CI, 0.68–1.01], respectively; P‐trend 0.0015). Having never smoked compared with being a current smoker tended to be associated with lower odds for CHIP.
Conclusions
A healthy lifestyle, based on a composite score, was not related to CHIP among postmenopausal women. However, across individual lifestyle factors, having a normal body mass index was strongly associated with a lower prevalence of CHIP. These findings support the idea that certain healthy lifestyle factors are associated with a lower frequency of CHIP.
The first description of neuromyelitis optica by Eugène Devic and Fernand Gault dates back to the 19th century, but only the discovery of aquaporin-4 autoantibodies in a major subset of affected patients in 2004 led to a fundamentally revised disease concept: Neuromyelits optica spectrum disorders (NMOSD) are now considered autoantibody-mediated autoimmune diseases, bringing the pivotal pathogenetic role of B cells and plasma cells into focus. Not long ago, there was no approved medication for this deleterious disease and off-label therapies were the only treatment options for affected patients. Within the last years, there has been a tremendous development of novel therapies with diverse treatment strategies: immunosuppression, B cell depletion, complement factor antagonism and interleukin-6 receptor blockage were shown to be effective and promising therapeutic interventions. This has led to the long-expected official approval of eculizumab in 2019 and inebilizumab in 2020. In this article, we review current pathogenetic concepts in NMOSD with a focus on the role of B cells and autoantibodies as major contributors to the propagation of these diseases. Lastly, by highlighting promising experimental and future treatment options, we aim to round up the current state of knowledge on the therapeutic arsenal in NMOSD.
Aims
This study aimed to identify echocardiographic determinants of left ventricular thrombus (LVT) formation after acute anterior myocardial infarction (MI).
Methods and results
This case–control study comprised 55 acute anterior MI patients with LVT as cases and 55 acute anterior MI patients without LVT as controls, who were selected from a cohort of consecutive patients with ischemic heart failure in our hospital. The cases and controls were matched for age, sex, and left ventricular ejection fraction. LVT was detected by routine/contrast echocardiography or cardiac magnetic resonance imaging during the first 3 months following MI. Formation of apical aneurysm after MI was independently associated with LVT formation [72.0% vs. 43.5%, odds ratio (OR) = 5.06, 95% confidence interval (CI) 1.65–15.48, P = 0.005]. Echocardiographic risk factors associated with LVT formation included reduced mitral annular plane systolic excursion (<7 mm, OR = 4.69, 95% CI 1.84–11.95, P = 0.001), moderate–severe diastolic dysfunction (OR = 2.71, 95% CI 1.11–6.57, P = 0.028), and right ventricular (RV) dysfunction [reduced tricuspid annular plane systolic excursion < 17 mm (OR = 5.48, 95% CI 2.12–14.13, P < 0.001), reduced RV fractional area change < 0.35 (OR = 3.32, 95% CI 1.20–9.18, P = 0.021), and enlarged RV mid diameter (per 5 mm increase OR = 1.62, 95% CI 1.12–2.34, P = 0.010)]. Reduced tricuspid annular plane systolic excursion (<17 mm) significantly associated with increased risk of LVT in anterior MI patients (OR = 3.84, 95% CI 1.37–10.75, P = 0.010), especially in those patients without apical aneurysm (OR = 5.12, 95% CI 1.45–18.08, P = 0.011), independent of body mass index, hypertension, anaemia, mitral annular plane systolic excursion, and moderate–severe diastolic dysfunction.
Conclusions
Right ventricular dysfunction as determined by reduced TAPSE or RV fractional area change is independently associated with LVT formation in acute anterior MI patients, especially in the setting of MI patients without the formation of an apical aneurysm. This study suggests that besides assessment of left ventricular abnormalities, assessment of concomitant RV dysfunction is of importance on risk stratification of LVT formation in patients with acute anterior MI.
Background
Treatment options for poorly differentiated (PDTC) and anaplastic (ATC) thyroid carcinoma are unsatisfactory and prognosis is generally poor. Lenvatinib (LEN), a multi-tyrosine kinase inhibitor targeting fibroblast growth factor receptors (FGFR) 1-4 is approved for advanced radioiodine refractory thyroid carcinoma, but response to single agent is poor in ATC. Recent reports of combining LEN with PD-1 inhibitor pembrolizumab (PEM) are promising.
Materials and Methods
Primary ATC (n=93) and PDTC (n=47) tissue samples diagnosed 1997-2019 at five German tertiary care centers were assessed for PD-L1 expression by immunohistochemistry using Tumor Proportion Score (TPS). FGFR 1-4 mRNA was quantified in 31 ATC and 14 PDTC with RNAscope in-situ hybridization. Normal thyroid tissue (NT) and papillary thyroid carcinoma (PTC) served as controls. Disease specific survival (DSS) was the primary outcome variable.
Results
PD-L1 TPS≥50% was observed in 42% of ATC and 26% of PDTC specimens. Mean PD-L1 expression was significantly higher in ATC (TPS 30%) than in PDTC (5%; p<0.01) and NT (0%, p<0.001). 53% of PDTC samples had PD-L1 expression ≤5%. FGFR mRNA expression was generally low in all samples but combined FGFR1-4 expression was significantly higher in PDTC and ATC compared to NT (each p<0.001). No impact of PD-L1 and FGFR 1-4 expression was observed on DSS.
Conclusion
High tumoral expression of PD-L1 in a large proportion of ATCs and a subgroup of PDTCs provides a rationale for immune checkpoint inhibition. FGFR expression is low thyroid tumor cells. The clinically observed synergism of PEM with LEN may be caused by immune modulation.
Chronotype is defined as the behavioral manifestation of circadian rhythms related to the external light–dark cycle. Evening chronotype has been associated with an increased risk of developing cardiometabolic diseases in obesity. Menopause is a lifestage associated with an increased risk of developing cardiometabolic diseases and a change in circadian rhythmicity compared to pre-menopause. However, the prevalence of chronotype categories in menopause and their role in determining menopause-related cardiometabolic risk, mostly in obesity, have not been investigated. Thus, we aimed to investigate the prevalence of chronotype categories in post-menopausal women with obesity and their role in menopause-related cardiometabolic risk. In this cross-sectional study we enrolled 49 pre-menopausal and 74 post-menopausal women with obesity. Anthropometric parameters, lifestyle habits, adherence to the Mediterranean Diet (MD), sleep quality, chronotype and the presence of type 2 diabetes mellitus (T2DM) and cardiovascular diseases (CVD) were studied. No significance differences were detected in terms of lifestyle and adherence to the MD between pre- and post-menopausal women. Chronotype was classified as morning in 66 (53.6%), evening in 20 (16.3%) and intermediate in 37 (30.1%) women. In addition, pre-menopausal women with obesity showed a significantly higher chance to have an intermediate chronotype (OR = 2.21, 95% CI 1.28–3.83; p = 0.004), whereas post-menopausal women with obesity showed a trend to have a higher morning chronotype (OR = 1.42, 95% CI 0.98–2.06; p = 0.051), although this did not reach statistical significance. No significant differences were detected in terms of prevalence of evening chronotype between the two groups. However, the evening chronotype had a significantly higher risk to have T2DM compared to the morning (OR = 17.29, 95% CI 2.40–124.27; p = 0.005) and intermediate chronotypes (OR = 30.86, 95% CI 2.05–464.32; p = 0.013) in both pre- and post-menopausal women with obesity. In conclusion, the intermediate chronotype was significantly more prevalent in pre-menopausal women with obesity compared to post-menopausal women. Evening chronotype was associated to T2DM in both pre- and post-menopause. These results support the importance of including the assessment of chronotype in the management of women with obesity in post-menopause.
Activation of the complement system and leukocytes by blood–membrane interactions may further promote arteriosclerosis typically present in patients on lipoprotein apheresis. As clinical data on the hemocompatibility of lipoprotein apheresis are scarce, a controlled clinical study comparing two different types of plasma separation and fractionation membranes used in double-filtration lipoprotein apheresis was urgently needed, as its outcome may influence clinical decision-making. In a prospective, randomized, crossover controlled trial, eight patients on double-filtration lipoprotein apheresis were subjected to one treatment with recent polyethersulfone (PES) plasma separation and fractionation membranes and one control treatment using a set of ethylene-vinyl alcohol copolymer (EVAL) membranes. White blood cell (WBC) and platelet (PC) counts, complement factor C5a and thrombin–antithrombin III (TAT) concentrations were determined in samples drawn at defined times from different sites of the extracorporeal blood and plasma circuit. With a nadir at 25 minutes, WBCs in EVAL decreased to 33.5 ± 10.7% of baseline compared with 63.8 ± 22.0% at 20 minutes in PES (P < .001). The maximum C5a levels in venous blood reentering the patients were measured at 30 minutes, being 30.0 ± 11.2 µg/L with EVAL and 12.3 ± 9.0 µg/L with PES (P < .05). The highest C5a concentrations were found in plasma after the plasma filters (EVAL 56.1 ± 22.0 µg/L at 15 minutes vs PES 23.3 ± 15.2 µg/L at 10 minutes; P < .001). PC did not significantly decrease over time with both membrane types, whereas TAT levels did not rise until the end of the treatment without differences between membranes. Regarding lipoprotein(a) and low-density lipoprotein (LDL) cholesterol removal, both membrane sets performed equally. Compared with EVAL, PES membranes cause less leukocyte and complement system activation, the classical parameters of hemocompatibility of extracorporeal treatment procedures, at identical treatment efficacy. Better hemocompatibility may avoid inflammation-promoting effects through blood–material interactions in patients requiring double-filtration lipoprotein apheresis.
Background: The hypothalamus is an important brain region for the regulation of energy balance. Roux-en-Y gastric bypass (RYGB) surgery and gut hormone-based treatments are known to reduce body weight, but their effects on hypothalamic gene expression and signaling pathways are poorly studied. Methods: Diet-induced obese male Wistar rats were randomized into the following groups: RYGB, sham operation, sham + body weight-matched (BWM) to the RYGB group, osmotic minipump delivering PYY3-36 (0.1 mg/kg/day), liraglutide s.c. (0.4 mg/kg/day), PYY3-36 + liraglutide, and saline. All groups (except BWM) were kept on a free choice of high- and low-fat diets. Four weeks after interventions, hypothalami were collected for RNA sequencing. Results: While rats in the RYGB, BWM, and PYY3-36 + liraglutide groups had comparable reductions in body weight, only RYGB and BWM treatment had a major impact on hypothalamic gene expression. In these groups, hypothalamic leptin receptor expression as well as the JAK–STAT, PI3K-Akt, and AMPK signaling pathways were upregulated. No significant changes could be detected in PYY3-36 + liraglutide-, liraglutide-, and PYY-treated groups. Conclusions: Despite causing similar body weight changes compared to RYGB and BWM, PYY3-36 + liraglutide treatment does not impact hypothalamic gene expression. Whether this striking difference is favorable or unfavorable to metabolic health in the long term requires further investigation.
The cardiovascular and immune systems undergo profound and intertwined alterations with aging. Recent studies have reported that an accumulation of memory and terminally differentiated T cells in elderly subjects can fuel myocardial aging and boost the progression of heart diseases. Nevertheless, it remains unclear whether the immunological senescence profile is sufficient to cause age-related cardiac deterioration or merely acts as an amplifier of previous tissue-intrinsic damage. Herein, we sought to decompose the causality in this cardio-immune crosstalk by studying young mice harboring a senescent-like expanded CD4\(^+\) T cell compartment. Thus, immunodeficient NSG-DR1 mice expressing HLA-DRB1*01:01 were transplanted with human CD4\(^+\) T cells purified from matching donors that rapidly engrafted and expanded in the recipients without causing xenograft reactions. In the donor subjects, the CD4\(^+\) T cell compartment was primarily composed of naïve cells defined as CCR7\(^+\)CD45RO\(^-\). However, when transplanted into young lymphocyte-deficient mice, CD4\(^+\) T cells underwent homeostatic expansion, upregulated expression of PD-1 receptor and strongly shifted towards effector/memory (CCR7\(^-\) CD45RO\(^+\)) and terminally-differentiated phenotypes (CCR7\(^-\)CD45RO\(^-\)), as typically seen in elderly. Differentiated CD4\(^+\) T cells also infiltrated the myocardium of recipient mice at comparable levels to what is observed during physiological aging. In addition, young mice harboring an expanded CD4\(^+\) T cell compartment showed increased numbers of infiltrating monocytes, macrophages and dendritic cells in the heart. Bulk mRNA sequencing analyses further confirmed that expanding T-cells promote myocardial inflammaging, marked by a distinct age-related transcriptomic signature. Altogether, these data indicate that exaggerated CD4\(^+\) T-cell expansion and differentiation, a hallmark of the aging immune system, is sufficient to promote myocardial alterations compatible with inflammaging in juvenile healthy mice.
Therapy options for adrenal insufficiency and recommendations for the management of adrenal crisis

Adrenal insufficiency (AI) is a life-threatening condition requiring life-long glucocorticoid (GC) substitution therapy, as well as stress adaptation to prevent adrenal crises. The number of individuals with primary and secondary adrenal insufficiency in Europe is estimated to be 20–50/100.000. A growing number of AI cases are due to side effects of GC treatment used in different treatment strategies for cancer and to immunotherapy in cancer treatment. The benefit of hormone replacement therapy is evident but long-term adverse effects may arise due to the non-physiological GC doses and treatment regimens used. Given multiple GC replacement formulations available comprising short-acting, intermediate, long-acting and novel modified-release hydrocortisone as well as subcutaneous formulations, this review offers a concise summary on the latest therapeutic improvements for treatment of AI and prevention of adrenal crises. As availability of various glucocorticoid formulations and access to expert centers across Europe varies widely, European Reference Networks on rare endocrine conditions aim at harmonizing treatment and ensure access to specialized patient care for individual case-by-case treatment decisions. To improve the availability across Europe to cost effective oral and parenteral formulations of hydrocortisone will save lives.
Metastatic pheochromocytoma and paraganglioma: signs and symptoms related to catecholamine secretion

Background
The presence or future development of metastatic pheochromocytomas or paragangliomas (mPPGLs) can be difficult to diagnose or predict at initial presentation. Since production of catecholamines from mPPGLs is different from non-metastatic tumors (non-mPPGLs), this study aimed to clarify whether presenting catecholamine-related signs and symptoms (cSS) might also differ.
Methods
The study included 249 patients, 43 with mPPGL and 206 with non-mPPGL. Clinical data at the time of biochemical diagnosis (i.e. at entry into the study) were used to generate a cumulative score of cSS for each patient.
Results
Patients with mPPGL were significantly younger (43.3 ± 14 vs. 48.9 ± 16.1 years) and included a lower proportion of females (39.5% vs. 60.7%) than patients with non-mPPGLs. Frequencies of signs and symptoms did not differ between the two groups. Patients with mPPGLs had lower (P < 0.001) urinary excretion of epinephrine (3.5 (IQR, 1.9—6.5) µg/day) than those with non-mPPGLs (19.1 (IQR, 4.3—70.2) µg/day). There was no difference in urinary excretion of norepinephrine. In patients with mPPGLs a high cSS score was associated with high urinary excretion of norepinephrine and normetanephrine. In contrast, in patients with non-mPPGLs, a high cSS was associated with high urinary excretion of epinephrine and metanephrine.
Conclusion
Although presenting signs and symptoms were associated with production of norepinephrine in patients with mPPGLs and of epinephrine in patients with non-mPPGLs, there were no differences in signs and symptoms between the two groups. Therefore, consideration of signs and symptoms does not appear helpful for distinguishing patients with and without mPPGLs.
Purpose
The subclassification of adrenal cancers according to the WHO classification in ordinary, myxoid, oncocytic, and sarcomatoid as well as pediatric types is well established, but the criteria for each subtype are not sufficiently determined and the relative frequency of the different types of adrenal cancers has not been studied in large cohorts. Therefore, our large collection of surgically removed adrenal cancers should be reviewed o establish the criteria for the subtypes and to find out the frequency of the various types.
Methods
In our series of 521 adrenal cancers the scoring systems of Weiss et al., Hough et al., van Slooten et al. and the new Helsinki score system were used for the ordinary type of cancer (97% of our series) and the myxoid type (0.8%). For oncocytic carcinomas (2%), the scoring system of Bisceglia et al. was applied.
Results
Discrepancies between benign and malignant diagnoses from the first thee classical scoring systems are not rare (22% in our series) and could be resolved by the Helsinki score especially by Ki-67 index (more than 8% unequivocally malignant). Since all our cancer cases are positive in the Helsinki score, this system can replace the three elder systems. For identification of sarcomatoid cancer as rarest type in our series (0.2%), the scoring systems are not practical but additional immunostainings used for soft tissue tumors and in special cases molecular pathology are necessary to differentiate these cancers from adrenal sarcomas. According to the relative frequencies of the different subtypes of adrenal cancers the main type is the far most frequent (97%) followed by the oncocytic type (2%), the myxoid type (0.8%) and the very rare sarcomatoid type (0.2%).
Conclusions
The Helsinki score is the best for differentiating adrenal carcinomas of the main, the oncocytic, and the myxoid type in routine work. Additional scoring systems for these carcinomas are generally not any longer necessary. Signs of proliferation (mitoses and Ki-67 index) and necroses are the most important criteria for diagnosis of malignancy.
A deep integration of routine care and research remains challenging in many respects. We aimed to show the feasibility of an automated transformation and transfer process feeding deeply structured data with a high level of granularity collected for a clinical prospective cohort study from our hospital information system to the study's electronic data capture system, while accounting for study-specific data and visits. We developed a system integrating all necessary software and organizational processes then used in the study. The process and key system components are described together with descriptive statistics to show its feasibility in general and to identify individual challenges in particular. Data of 2051 patients enrolled between 2014 and 2020 was transferred. We were able to automate the transfer of approximately 11 million individual data values, representing 95% of all entered study data. These were recorded in n = 314 variables (28% of all variables), with some variables being used multiple times for follow-up visits. Our validation approach allowed for constant good data quality over the course of the study. In conclusion, the automated transfer of multi-dimensional routine medical data from HIS to study databases using specific study data and visit structures is complex, yet viable.
Adrenocortical tumors are rare in children. This systematic review summarizes the published evidence on pediatric adrenocortical carcinoma (ACC) to provide a basis for a better understanding of the disease, investigate new molecular biomarkers and therapeutic targets, and define which patients may benefit from a more aggressive therapeutic approach. We included 137 studies with 3680 ACC patients (~65% female) in our analysis. We found no randomized controlled trials, so this review mainly reflects retrospective data. Due to a specific mutation in the TP53 gene in ~80% of Brazilian patients, that cohort was analyzed separately from series from other countries. Hormone analysis was described in 2569 of the 2874 patients (89%). Most patients were diagnosed with localized disease, whereas 23% had metastasis at primary diagnosis. Only 72% of the patients achieved complete resection. In 334 children (23%), recurrent disease was reported: 81% — local recurrence, 19% (n = 65) — distant metastases at relapse. Patients < 4 years old had a different distribution of tumor stages and hormone activity and better overall survival (p < 0.001). Although therapeutic approaches are typically multimodal, no consensus is available on effective standard treatments for advanced ACC. Thus, knowledge regarding pediatric ACC is still scarce and international prospective studies are needed to implement standardized clinical stratifications and risk-adapted therapeutic strategies.
Anxiety disorders and depression are common comorbidities in cardiac patients. Mice lacking the serotonin transporter (5-HTT) exhibit increased anxiety-like behavior. However, the role of 5-HTT deficiency on cardiac aging, and on healing and remodeling processes after myocardial infarction (MI), remains unclear. Cardiological evaluation of experimentally naïve male mice revealed a mild cardiac dysfunction in ≥4-month-old 5-HTT knockout (−/−) animals. Following induction of chronic cardiac dysfunction (CCD) by MI vs. sham operation 5-HTT−/− mice with infarct sizes >30% experienced 100% mortality, while 50% of 5-HTT+/− and 37% of 5-HTT+/+ animals with large MI survived the 8-week observation period. Surviving (sham and MI < 30%) 5-HTT−/− mutants displayed reduced exploratory activity and increased anxiety-like behavior in different approach-avoidance tasks. However, CCD failed to provoke a depressive-like behavioral response in either 5-Htt genotype. Mechanistic analyses were performed on mice 3 days post-MI. Electrocardiography, histology and FACS of inflammatory cells revealed no abnormalities. However, gene expression of inflammation-related cytokines (TGF-β, TNF-α, IL-6) and MMP-2, a protein involved in the breakdown of extracellular matrix, was significantly increased in 5-HTT−/− mice after MI. This study shows that 5-HTT deficiency leads to age-dependent cardiac dysfunction and disrupted early healing after MI probably due to alterations of inflammatory processes in mice.
A liquid chromatography tandem mass spectrometry method for the analysis of ten kinase inhibitors (afatinib, axitinib, bosutinib,cabozantinib, dabrafenib, lenvatinib, nilotinib, osimertinib, ruxolitinib, and trametinib) in human serum and plasma for theapplication in daily clinical routine has been developed and validated according to the US Food and Drug Administration andEuropean Medicines Agency validation guidelines for bioanalytical methods. After protein precipitation of plasma samples withacetonitrile, chromatographic separation was performed at ambient temperature using a Waters XBridge® Phenyl 3.5μm(2.1×50 mm) column. The mobile phases consisted of water-methanol (9:1, v/v) with 10 mM ammonium bicarbonate as phase A andmethanol-water (9:1, v/v) with 10 mM ammonium bicarbonate as phase B. Gradient elution was applied at a flow rate of 400μL/min. Analytes were detected and quantified using multiple reaction monitoring in electrospray ionization positive mode. Stableisotopically labeled compounds of each kinase inhibitor were used as internal standards. The acquisition time was 7.0 min perrun. All analytes and internal standards eluted within 3.0 min. The calibration curves were linear over the range of 2–500 ng/mLfor afatinib, axitinib, bosutinib, lenvatinib, ruxolitinib, and trametinib, and 6–1500 ng/mL for cabozantinib, dabrafenib, nilotinib,and osimertinib (coefficients of correlation≥0.99). Validation assays for accuracy and precision, matrix effect, recovery,carryover, and stability were appropriate according to regulatory agencies. The rapid and sensitive assay ensures high throughputand was successfully applied to monitor concentrations of kinase inhibitors in patients.
Context
Cushing’s syndrome (CS) is a rare disease of endogenous hypercortisolism associated with high morbidity and mortality. Diagnosis and classification of CS is still challenging.
Objective
Circulating microRNAs (miRNAs) are minimally invasive diagnostic markers. Our aim was to characterize the circulating miRNA profiles of CS patients and to identify distinct profiles between the two major CS subtypes.
Methods
We included three groups of patients from the German Cushing’s registry: ACTH-independent CS (Cortisol-Producing-Adenoma; CPA), ACTH-dependent pituitary CS (Cushing’s Disease; CD), and patients in whom CS had been ruled out (controls). Profiling of miRNAs was performed by next-generation-sequencing (NGS) in serum samples of 15 CS patients (each before and after curative surgery) and 10 controls. Significant miRNAs were first validated by qPCR in the discovery cohort and then in an independent validation cohort of 20 CS patients and 11 controls.
Results
NGS identified 411 circulating miRNAs. Differential expression of 14 miRNAs were found in the pre- and postoperative groups. qPCR in the discovery cohort validated 5 of the significant miRNAs from the preoperative group analyses. Only, miR-182-5p was found to be significantly upregulated in the CD group of the validation cohort. Comparing all CS samples as a group with the controls did not reveal any significant differences in expression.
Outcome
In conclusion, our study identified miR-182-5p as a possible biomarker for CD, which has to be validated in a prospective cohort. Furthermore, our results suggest that presence or absence of ACTH might be at least as relevant for miRNA expression as hypercortisolism itself.
Simultaneous measurements of 3D wall shear stress and pulse wave velocity in the murine aortic arch

Purpose
Wall shear stress (WSS) and pulse wave velocity (PWV) are important parameters to characterize blood flow in the vessel wall. Their quantification with flow-sensitive phase-contrast (PC) cardiovascular magnetic resonance (CMR), however, is time-consuming. Furthermore, the measurement of WSS requires high spatial resolution, whereas high temporal resolution is necessary for PWV measurements. For these reasons, PWV and WSS are challenging to measure in one CMR session, making it difficult to directly compare these parameters. By using a retrospective approach with a flexible reconstruction framework, we here aimed to simultaneously assess both PWV and WSS in the murine aortic arch from the same 4D flow measurement.
Methods
Flow was measured in the aortic arch of 18-week-old wildtype (n = 5) and ApoE\(^{−/−}\) mice (n = 5) with a self-navigated radial 4D-PC-CMR sequence. Retrospective data analysis was used to reconstruct the same dataset either at low spatial and high temporal resolution (PWV analysis) or high spatial and low temporal resolution (WSS analysis). To assess WSS, the aortic lumen was labeled by semi-automatically segmenting the reconstruction with high spatial resolution. WSS was determined from the spatial velocity gradients at the lumen surface. For calculation of the PWV, segmentation data was interpolated along the temporal dimension. Subsequently, PWV was quantified from the through-plane flow data using the multiple-points transit-time method. Reconstructions with varying frame rates and spatial resolutions were performed to investigate the influence of spatiotemporal resolution on the PWV and WSS quantification.
Results
4D flow measurements were conducted in an acquisition time of only 35 min. Increased peak flow and peak WSS values and lower errors in PWV estimation were observed in the reconstructions with high temporal resolution. Aortic PWV was significantly increased in ApoE\(^{−/−}\) mice compared to the control group (1.7 ± 0.2 versus 2.6 ± 0.2 m/s, p < 0.001). Mean WSS magnitude values averaged over the aortic arch were (1.17 ± 0.07) N/m\(^2\) in wildtype mice and (1.27 ± 0.10) N/m\(^2\) in ApoE\(^{−/−}\) mice.
Conclusion
The post processing algorithm using the flexible reconstruction framework developed in this study permitted quantification of global PWV and 3D-WSS in a single acquisition. The possibility to assess both parameters in only 35 min will markedly improve the analyses and information content of in vivo measurements.
Purpose
Inhomogeneities of the static magnetic B\(_{0}\) field are a major limiting factor in cardiac MRI at ultrahigh field (≥ 7T), as they result in signal loss and image distortions. Different magnetic susceptibilities of the myocardium and surrounding tissue in combination with cardiac motion lead to strong spatio‐temporal B\(_{0}\)‐field inhomogeneities, and their homogenization (B0 shimming) is a prerequisite. Limitations of state‐of‐the‐art shimming are described, regional B\(_{0}\) variations are measured, and a methodology for spherical harmonics shimming of the B\(_{0}\) field within the human myocardium is proposed.
Methods
The spatial B\(_{0}\)‐field distribution in the heart was analyzed as well as temporal B\(_{0}\)‐field variations in the myocardium over the cardiac cycle. Different shim region‐of‐interest selections were compared, and hardware limitations of spherical harmonics B\(_{0}\) shimming were evaluated by calibration‐based B0‐field modeling. The role of third‐order spherical harmonics terms was analyzed as well as potential benefits from cardiac phase–specific shimming.
Results
The strongest B\(_{0}\)‐field inhomogeneities were observed in localized spots within the left‐ventricular and right‐ventricular myocardium and varied between systolic and diastolic cardiac phases. An anatomy‐driven shim region‐of‐interest selection allowed for improved B\(_{0}\)‐field homogeneity compared with a standard shim region‐of‐interest cuboid. Third‐order spherical harmonics terms were demonstrated to be beneficial for shimming of these myocardial B\(_{0}\)‐field inhomogeneities. Initial results from the in vivo implementation of a potential shim strategy were obtained. Simulated cardiac phase–specific shimming was performed, and a shim term‐by‐term analysis revealed periodic variations of required currents.
Conclusion
Challenges in state‐of‐the‐art B\(_{0}\) shimming of the human heart at 7 T were described. Cardiac phase–specific shimming strategies were found to be superior to vendor‐supplied shimming.
Aims
The role of diastolic dysfunction (DD) in prognostic evaluation in heart failure (HF) patients with impaired systolic function remains unclear. We investigated the impact of echocardiography-defined DD on survival in HF patients with mid-range (HFmrEF, EF 41–49%) and reduced ejection fraction (HFrEF, EF < 40%).
Methods and results
A total of 2018 consecutive hospitalized HF patients were retrospectively included and divided in two groups based on baseline EF: HFmrEF group (n = 951, aged 69 ± 13 years, 74.2% male) and HFrEF group (n = 1067, aged 68 ± 13 years, 76.3% male). Clinical data were collected and analysed. All patients completed ≥1 year clinical follow-up. The primary endpoint was defined as all-cause death (including heart transplantation) and cardiovascular (CV)-related death. All-cause mortality (30.8% vs. 24.9%, P = 0.003) and CV mortality (19.1% vs. 13.5%, P = 0.001) were significantly higher in the HFrEF group than the HFmrEF group during follow-up [median 24 (13–36) months]. All-cause mortality increased in proportion to DD severity (mild, moderate, and severe) in either HFmrEF (17.1%, 25.4%, and 37.0%, P < 0.001) or HFrEF (18.9%, 30.3%, and 39.2%, P < 0.001) patients. The risk of all-cause mortality [hazard ratio (HR) = 1.347, P = 0.015] and CV mortality (HR = 1.508, P = 0.007) was significantly higher in HFrEF patients with severe DD compared with non-severe DD after adjustment for identified clinical and echocardiographic covariates. For HFmrEF patients, severe DD was independently associated with increased all-cause mortality (HR = 1.358, P = 0.046) but not with CV mortality (HR = 1.155, P = 0.469).
Conclusions
Echocardiography-defined severe DD is independently associated with increased all-cause mortality in patients with HFmrEF and HFrEF.
About 50% of patients with arrhythmogenic cardiomyopathy (ACM) carry a pathogenic or likely pathogenic mutation in the desmosomal genes. However, there is a significant number of patients without positive familial anamnesis. Therefore, the molecular reasons for ACM in these patients are frequently unknown and a genetic contribution might be underestimated. Here, we used a next-generation sequencing (NGS) approach and in addition single nucleotide polymor-phism (SNP) arrays for the genetic analysis of two independent index patients without familial medical history. Of note, this genetic strategy revealed a homozygous splice site mutation (DSG2–c.378+1G>T) in the first patient and a nonsense mutation (DSG2–p.L772X) in combination with a large deletion in DSG2 in the second one. In conclusion, a recessive inheritance pattern is likely for both cases, which might contribute to the hidden medical history in both families. This is the first report about these novel loss-of-function mutations in DSG2 that have not been previously identi-fied. Therefore, we suggest performing deep genetic analyses using NGS in combination with SNP arrays also for ACM index patients without obvious familial medical history. In the future, this finding might has relevance for the genetic counseling of similar cases.
Aims
It has been hypothesized that cardiac decompensation accompanying acute heart failure (AHF) episodes generates a pro-inflammatory environment boosting an adaptive immune response against myocardial antigens, thus contributing to progression of heart failure (HF) and poor prognosis. We assessed the prevalence of anti-myocardial autoantibodies (AMyA) as biomarkers reflecting adaptive immune responses in patients admitted to the hospital for AHF, followed the change in AMyA titres for 6 months after discharge, and evaluated their prognostic utility.
Methods and results
AMyA were determined in n = 47 patients, median age 71 (quartiles 60; 80) years, 23 (49%) female, and 24 (51%) with HF with preserved ejection fraction, from blood collected at baseline (time point of hospitalization) and at 6 month follow-up (visit F6). Patients were followed for 18 months (visit F18). The prevalence of AMyA increased from baseline (n = 21, 45%) to F6 (n = 36, 77%; P < 0.001). At F6, the prevalence of AMyA was higher in patients with HF with preserved ejection fraction (n = 21, 88%) compared with patients with reduced ejection fraction (n = 14, 61%; P = 0.036). During the subsequent 12 months after F6, that is up to F18, patients with newly developed AMyA at F6 had a higher risk for the combined endpoint of death or rehospitalization for HF (hazard ratio 4.79, 95% confidence interval 1.13–20.21; P = 0.033) compared with patients with persistent or without AMyA at F6.
Conclusions
Our results support the hypothesis that AHF may induce patterns of adaptive immune responses. More studies in larger populations and well-defined patient subgroups are needed to further clarify the role of the adaptive immune system in HF progression.
Functional versus morphological assessment of vascular age in patients with coronary heart disease

Communicating cardiovascular risk based on individual vascular age (VA) is a well acknowledged concept in patient education and disease prevention. VA may be derived functionally, e.g. by measurement of pulse wave velocity (PWV), or morphologically, e.g. by assessment of carotid intima-media thickness (cIMT). The purpose of this study was to investigate whether both approaches produce similar results. Within the context of the German subset of the EUROASPIRE IV survey, 501 patients with coronary heart disease underwent (a) oscillometric PWV measurement at the aortic, carotid-femoral and brachial-ankle site (PWVao, PWVcf, PWVba) and derivation of the aortic augmentation index (AIao); (b) bilateral cIMT assessment by high-resolution ultrasound at three sites (common, bulb, internal). Respective VA was calculated using published equations. According to VA derived from PWV, most patients exhibited values below chronological age indicating a counterintuitive healthier-than-anticipated vascular status: for VA(PWVao) in 68% of patients; for VA\(_{AIao}\) in 52% of patients. By contrast, VA derived from cIMT delivered opposite results: e.g. according to VA\(_{total-cIMT}\) accelerated vascular aging in 75% of patients. To strengthen the concept of VA, further efforts are needed to better standardise the current approaches to estimate VA and, thereby, to improve comparability and clinical utility.
Die Transkatheter-Aortenklappenimplantation (TAVI) ist eine etablierte Prozedur zur Therapie der Aortenklappenstenose. Bei der transfemoralen TAVI sind Gefäßkomplikationen am Punktionsort weiterhin ein Problem und verantwortlich für einen Hauptteil der Komplikationen. Um eine sichere Punktion in einem nicht-kalzifizierten Gefäßabschnitt sicherzustellen, wurde eine neue Navigationstechnik mit Ultraschallunterstützung verwendet. Wir verglichen 67 konsekutive TAVI-Patienten mit Navigations- und Ultraschall-gestützter Punktion der Leistenarterie mit 67 konsekutiven Patienten mit Fluoroskopischer Punktion.
Simple Summary
Using a visual-based clustering method on the TCGA RNA sequencing data of a large adrenocortical carcinoma (ACC) cohort, we were able to classify these tumors in two distinct clusters largely overlapping with previously identified ones. As previously shown, the identified clusters also correlated with patient survival. Applying the visual clustering method to a second dataset also including benign adrenocortical samples additionally revealed that one of the ACC clusters is more closely located to the benign samples, providing a possible explanation for the better survival of this ACC cluster. Furthermore, the subsequent use of machine learning identified new possible biomarker genes with prognostic potential for this rare disease, that are significantly differentially expressed in the different survival clusters and should be further evaluated.
Abstract
Adrenocortical carcinoma (ACC) is a rare disease, associated with poor survival. Several “multiple-omics” studies characterizing ACC on a molecular level identified two different clusters correlating with patient survival (C1A and C1B). We here used the publicly available transcriptome data from the TCGA-ACC dataset (n = 79), applying machine learning (ML) methods to classify the ACC based on expression pattern in an unbiased manner. UMAP (uniform manifold approximation and projection)-based clustering resulted in two distinct groups, ACC-UMAP1 and ACC-UMAP2, that largely overlap with clusters C1B and C1A, respectively. However, subsequent use of random-forest-based learning revealed a set of new possible marker genes showing significant differential expression in the described clusters (e.g., SOAT1, EIF2A1). For validation purposes, we used a secondary dataset based on a previous study from our group, consisting of 4 normal adrenal glands and 52 benign and 7 malignant tumor samples. The results largely confirmed those obtained for the TCGA-ACC cohort. In addition, the ENSAT dataset showed a correlation between benign adrenocortical tumors and the good prognosis ACC cluster ACC-UMAP1/C1B. In conclusion, the use of ML approaches re-identified and redefined known prognostic ACC subgroups. On the other hand, the subsequent use of random-forest-based learning identified new possible prognostic marker genes for ACC.
The occurrence of different subtypes of endogenous Cushing’s syndrome (CS) in single individuals is extremely rare. We here present the case of a female patient who was successfully cured from adrenal CS 4 years before being diagnosed with Cushing’s disease (CD). The patient was diagnosed at the age of 50 with ACTH-independent CS and a left-sided adrenal adenoma, in January 2015. After adrenalectomy and histopathological confirmation of a cortisol-producing adrenocortical adenoma, biochemical hypercortisolism and clinical symptoms significantly improved. However, starting from 2018, the patient again developed signs and symptoms of recurrent CS. Subsequent biochemical and radiological workup suggested the presence of ACTH-dependent CS along with a pituitary microadenoma. The patient underwent successful transsphenoidal adenomectomy, and both postoperative adrenal insufficiency and histopathological workup confirmed the diagnosis of CD. Exome sequencing excluded a causative germline mutation but showed somatic mutations of the β-catenin protein gene (CTNNB1) in the adrenal adenoma, and of both the ubiquitin specific peptidase 8 (USP8) and the glucocorticoid receptor (NR3C1) genes in the pituitary adenoma. In conclusion, our case illustrates that both ACTH-independent and ACTH-dependent CS may develop in a single individual even without evidence for a common genetic background.
Background: Endovascular therapy is the gold standard in patients with hemodynamic relevant renal artery stenosis (RAS) resistant to medical therapy. The severity grading of the stenosis as well as the result assessment after endovascular approach is predominantly based on visible estimations of the anatomic appearance. We aim to investigate the application of color-coded DSA parameters to gain hemodynamic information during endovascular renal artery interventions and for the assessment of the procedures technical success.
Methods: We retrospectively evaluated 32 patients who underwent endovascular renal artery revascularization and applied color-coded summation imaging on selected monochromatic DSA images. The differences in time to peak (dTTP) of contrast enhancement in predefined anatomical measuring points were analyzed. Furthermore, differences in systolic blood pressure values (SBP) and serum creatinine were obtained. The value of underlying diabetes mellitus as a predictor for clinical outcome was assessed. Correlation analysis between the patients gender as well as the presence of diabetes mellitus and dTTP was performed.
Results: Endovascular revascularization resulted in statistically significant improvement in 4/7 regions of interest. Highly significant improvement of perfusion in terms of shortened TTP values could be found at the segmental artery level and in the intrastenotical segment (p<0.001), significant improvement prestenotical and in the apical renal parenchyma (p<0.05). In the other anatomic regions, differences revealed not to be significant. Differences between SBP and serum creatinine levels before and after the procedure were significant (p=0.004 and 0.0004). Patients ' gender as well as the presence of diabetes mellitus did not reveal to be predictors for the clinical success of the procedure. Furthermore, diabetes and gender did not show relevant correlation with dTTP in the parenchymal measuring points.
Conclusions: The supplementary use of color-coding DSA and the data gained from parametric images may provide helpful information in the evaluation of the procedures ' technical success. The segmental artery might be a particularly suitable vascular territory for analyzing differences in blood flow characteristics. Further studies with larger cohorts are needed to further confirm the diagnostic value of this technique.
Rezente Studien mit kleineren Fallzahlen offenbaren bei Patienten mit chronischer Nebenniereninsuffizienz eine sehr negative Krankheitswahrnehmung, große Ängste und Sorgen hinsichtlich der Substitutionstherapie mit Glucocorticoiden sowie eine geringe Therapieadhärenz.
Ziel der vorliegenden Beobachtungsstudie war es daher im Rahmen einer monozentrischen Querschnittstudie nebenniereninsuffiziente Patienten zu Therapieadhärenz, subjektiver Krankheits- und Glucocorticoidwahrnehmung und Zufriedenheit mit erhaltenen Informationen zu befragen. Zudem wurden erstmalig die Zusammenhänge zwischen der Teilnahme an einer standardisierten NNI-Schulung und oben genannten Aspekten im Rahmen einer multizentrischen Längschnittstudie untersucht.
Die Ergebnisse der Querschnittstudie zeichnen insgesamt ein deutlich positiveres Bild von der subjektiven Krankheits- und Therapiewahrnehmung als bisher in der Literatur beschrieben. Die subjektive Therapieadhärenz war hoch. Zudem waren Sorgen und Ängste hinsichtlich der Glucocorticoid-Substitution geringer ausgeprägt als erwartet. Nichtsdestotrotz ließ sich konkordant zu früheren Publikationen eine zum Teil sehr große Unzufriedenheit mit erhaltenen Informationen zu möglichen Problemen der Glucocorticoid-Substitution feststellen. Die Ergebnisse der Längschnittstudie deuten darauf hin, dass die standardisierte Patientenschulung ein geeignetes Instrument sein könnte, um die Zufriedenheit von Patienten mit NNI zu steigern, das Selbstmanagement zu stärken und gleichzeitig positiven Einfluss auf die Wahrnehmung der Substitutionstherapie nehmen könnte.
MC ist eine seltene, lebensbedrohliche Erkrankung mit einer bisher unvollständig verstandenen Pathogenese. Die zugrundeliegenden ACTH-produzierenden Adenome beherbergen in 35-62% somatische Mutationen imUSP8 Gen. Die aktivierenden Mutationen machen USP8 hyperaktiv und verhindern damit den Abbau von Zielproteinen. Ziel dieser Studie war es, den Einfluss von USP8-Mutationen auf Proteine, die in MC dereguliert sind, zu identifizieren. Zudem wird untersucht, ob es Zusammenhänge der klinischen Daten bei Patienten mit bekanntem Mutationsstatus gibt. Für die Analyse der klinischen Daten wurden Patienten eingeschlossen, die sich zwischen 1997 und 2016 einer transsphenoidalen Operation in der Uniklinik Hamburg, Würzburg oder Erlangen unterzogen haben. Die Daten wurden von 125 unterschiedlichen Patienten mit 127 corticotropen Tumoren ausgewertet (34 USP8 mutiert, 93 USP8 WT). Von dem obengenannten Patientenkollektiv der ACTH-produzierenden Tumore für die Auswertung der klinischen Daten, lagen uns insgesamt Proben von 78 dieser Tumore zur immunhistochemischen Färbung vor. Zudem wurde weitere Proben von Hypophysenadenomen und normalen Hypophysen als Vergleichsgruppe gefärbt und untersucht. Insgesamt wurden 111 Hypophysenadenome sowie 5 normale Hypophysen untersucht. Die bereits mit MC assoziierten deregulierten Proteine (USP8, EGFR, P27/Kip1, Cyclin E, CRHR1,AVPR1b, TR4, HSP90, CREB, pCREB, PKA) wurden immunhistochemisch untersucht. Während bei den USP8 mutierten Tumoren die Patienten zu 100% weiblich waren, ergab die relative Häufigkeit der Frauen bei den WT Tumoren nur 62%.Die Patienten mit USP8 Mutation waren bei der ED des MC im Schnitt 10 Jahre jünger als die vom WT Der basale präoperative Cortisolspiegel zeigte sich signifikant höher in mutierten Tumoren und die ACTH-Spiegel ergaben ähnliche Werte bei WT und mutierten Hypophysenadenomen. Die Tumorgröße war signifikant kleiner in den USP8mutierten Adenomen. Während die Invasivitätsrate der Tumore sich ähnlich zeigte, lag bei den WT Adenomen signifikant häufiger eine subtotale Resektion des Tumors vor als bei den mutierten Tumoren vor. Die Rezidivrate ergab keinen signifikanten Unterschied. Während die meisten der mittels Immunhistochemie untersuchten Proteine keinen Unterschied in der Expression zwischen USP8 mutierten und WT Tumoren zeigten, war der Zellzyklusinhibitor p27/Kip1 signifikant reduziert in mutierten Tumoren Im Gegensatz dazu, zeigte sich das Chaperon HSP90 im Nukleus stärker exprimiert und die Phosphorylierung des Transkriptionsfaktor CREB war erhöht in mutierten Tumoren im Vergleich zum WT .Das lässt vermuten, dass p27/Kip1, HSP90 und pCREB direkte oder indirekte Zielproteine von USP8 sind und daher potenzielle Ziele für therapeutische Ansätze bei Patienten mit MC sein könnten.
Das Thema dieser Dissertation lautet „Konzeption und Evaluation eines webbasierten Patienteninformationsprogrammes zur Überprüfung internistischer Verdachtsdiagnosen“. Zusammen mit dem Institut für Informatik wurde das wissensbasierte second-opinion-System SymptomCheck entwickelt. Das Programm dient zur Überprüfung von Verdachtsdiagnosen. Es wurden Wissensbasen erstellt, in denen Symptome, Befunde und Untersuchungen nach einem Bewertungsschema beurteilt werden. Folgend wurde eine online erreichbare Startseite erstellt, auf der Nutzer vornehmlich internistische Verdachtsdiagnosen überprüfen können. Das Programm wurde in zwei Studien bezüglich seiner Sensitivität und Spezifität sowie der Benutzerfreundlichkeit getestet. In der ersten Studie wurden die Verdachtsdiagnosen ambulanter Patienten mit den ärztlich gestellten Diagnosen verglichen, eine zweite an die Allgemeinbevölkerung gerichtete Onlinestudie galt vor allem der Bewertung der Benutzerfreundlichkeit. Soweit bekannt ist dies die erste Studie in der ein selbst entwickeltes Programm selbstständig an echten Patienten getestet wurde.
Patienten mit fortgeschrittenem Morbus Fabry neigen zu einer Vielzahl an kardiologischen Symptomen, u.A. Herzrhythmusstörungen. Diese sind prognostisch natürlich relevant. Um diese detektieren zu können, erhielten unsere Patienten implantierbare Loop-Recorder. Schon in der ersten Studie konnten so einige signifikante Herzrhythmusereignisse diagnostiziert und eine klinische Konsequenz daraus gezogen werden. Diese Studie stellt nun ein 2-Jahres-Follow-Up zur ersten Studie dar.
Die vorliegende Arbeit befasst sich mit der Häufigkeit eines postoperativen Dumping-Syndroms (DS) bei Roux-en-Y gastric bypass (RYGB) und Sleeve-Gastrektomie (SG) Patienten. Primärer Endpunkt der Studie war die Häufigkeit der postoperativen Hypoglykämie (Spätdumping, definiert als Blutzucker < 50 mg/dl mit typischen Beschwerden oder < 40 mg/dl mit oder ohne typische Beschwerden) im modifizierten OGTT (mod. OGTT). Vorab wurde dieser mod. OGTT, der sich vom Standard-OGTT bei gleicher absoluter Glucosemenge nur im reduzierten Volumen (100 ml statt 300 ml) unterschied, validiert. Hierzu wurden beide Tests bei 10 gesunden Probanden durchgeführt. Es wurden insgesamt 30 RYGB Patienten und 12 SG Patienten in die Studie eingeschlossen. Insgesamt zeigt die vorliegende Arbeit, dass ein Jahr postoperativ eine relevante Anzahl von RYGB und SG Patienten ein Frühdumping im mod. OGTT aufwies und über Dumpingbeschwerden im Alltag berichtete. Während ein Frühdumping bei RYGB und SG Patienten gleichermaßen vorkam, wurden Hypoglykämien nur bei RYGB Patienten festgestellt. Der mod. OGTT scheint hierbei zur Detektion von DS Patienten, aufgrund seines geringen Volumens, eine gute Alternative zu den bisherigen diagnostischen Verfahren darzustellen. Der 13C-Octanoat-Atemgastest ermöglichte - auf Basis unserer Ergebnisse - keine eindeutige Abgrenzung der Dumping-Patienten von den Non-Dumpern.
Untersuchungen zum Einfluss der intraoperativen Schocktestung und weiterer Variablen auf die Schockeffektivität von implantierbaren Kardioverter Defibrillatoren. Studienkollektiv n=309, Datenerhebungszeitraum 2000 bis 2013, Vergleich einer ineffektiven und effektiven Schockgruppe hinsichtlich verschiedener Variablen.
Die Nicotinamid-N-Methyltransferase (NNMT) ist ein vor kurzem neu erkannter Regulator der Energiehomöostase im Fettgewebe. Die Hemmung von NNMT durch 1-Methylnicotinamid (1-MN) führt dosisabhängig zu einer Steigerung der Glycerolfreisetzung aus 3T3-L1-Adipozyten im Sinne einer gesteigerten Lipolyse. Die Sekretion von Adiponektin und Leptin wird durch 1-Methylnicotinamid nicht messbar verändert. Um die Messwerte unterschiedlich stark ausdifferenzierter Zellkulturpassagen miteinander vergleichen zu können, wurde eine photometrische Methode entwickelt und publiziert. Die Ergebnisse dieser Dissertation zeigen, dass NNMT die Lipolyse in Fettzellen reguliert, was zum besseren Verständnis der physiologischen Funktion dieser im Fettgewebe neu entdeckten Methyltransferase beiträgt.
Hintergrund:
Anlass der hier vorliegenden Dissertation ist die Neudefinition der ESC-Leitlinie zur Therapie der Herzinsuffizienz aus dem Jahr 2016. Neben den bereits klassifizierten Gruppen HFpEF (LVEF > 50 %) und HFrEF (LVEF < 40 %) wurde eine weitere HI-Gruppe bei mäßiggradig reduzierter linksventrikulärer Ejektionsfraktion im Bereich von 40 bis 49 % definiert (HFmrEF).
Fragestellung:
Die vorliegende Arbeit beschäftigte sich deshalb zum einen mit der Frage, inwiefern es sich bei den neudefinierten HFmrEF- im Vergleich zur bereits etablierten HFrEF-Gruppe, um eine klinisch sowie echokardiografisch eigenständige Patientengruppe handelt und zum anderen, ob Patienten der HFrEF- und HFmrEF-Gruppe prognostische Determinanten auf das Zweijahresüberleben aufweisen.
Methode:
Insgesamt wurden 804 Patienten mit einer linksventrikulären Ejektionsfraktion kleiner 50 % im Rahmen der Studie ausgewertet. Alle Patienten wurden am echokardiografischen Labor des Universitätsklinikums Würzburg untersucht und weiterbetreut. Der Erhebungszeitraum der klinisch-retrospektiv designten Beobachtungsstudie war von Juni 2009 bis Dezember 2014. Die Gesamtpatientengruppe wurde dabei in zwei Teilkollektive, die HFmrEF- (n = 389) und HFrEF-Gruppe (n = 415), untergliedert. Im Rahmen der Studie wurden klinische Patientendaten ausgewertet. Dazu wurde die Medizinhistorie (z. B. Begleiterkrankungen und Medikation), die Laborwerte und die echokardiografischen Parameter der Betroffenen ausgewertet. Die Studienpatienten wurden im zweiten Schritt über einen Follow-Up-Zeitraum von 24 Monaten (23,0 Monate ± 3,1 Monate) durch klinische Untersuchungen am UKW oder Telefoninterviews weiterbetreut. Der Studienendpunkt war das Versterben (all-cause death) oder die Herztransplantation eines Patienten. Die Zielsetzung der Arbeit, die Eigenständigkeit, sowie die Detektion einiger Determinanten auf das Zweijahresüberleben der Vergleichsgruppen herauszuarbeiten, konnte durch eine Reihe von Ergebniswerten für die hier vorliegenden Studienkollektive gezeigt werden.
Ergebnisse:
Das durchschnittliche Alter des Gesamtkollektives lag bei 67 ± 12 Lebensjahren, wobei 74,8 % der Patienten männlichen Geschlechts waren. Der Anteil an Dyslipidämie, Anämie und KHK war bei den Erkrankten des HFmrEF-Kollektivs signifikant häufiger. Betroffene der HFrEF-Gruppe zeigten hingegen einen signifikant höheren Anteil an Lungenerkrankungen und ICD bzw. Schrittmacher-Implantationsraten auf. Unter Berücksichtigung der Medikamenteneinnahme präsentierten sich im Vergleich signifikant höhere Einnahmeraten bei der HFrEF-Gruppe bei den Wirkstoffklassen der ACE-Hemmer, der Beta-Blocker, der Aldosteronrezeptorantagonisten, der Diuretika und der Digitalisglykoside. Die Einnahme von Statinen und Antikoagulantien war hingegen bei der neudefinierten Gruppe der HFmrEF-Patienten signifikant häufiger. Im Rahmen der Untersuchung des Blutes konnten zudem für die HFmrEF-Gruppe signifikant niedrigere NT-proBNP-Serumspiegel errechnet werden (940 vs. 1.760 pg / ml, p < 0,001). Unter Betrachtung der echokardiografischen Parameter zeigte sich außerdem für die HFrEF- im Vergleich zur HFmrEF-Patientengruppe eine signifikant höhere Prävalenz der kardialen Dilatation, der diastolischen Dysfunktion (moderate und schwere Ausprägung: 57,4 % vs. 40,6 %, p > 0,001), des Mitralklappenrückflusses (moderate und schwere Ausprägung: 17,4 % vs. 9 %, p = 0,006) und des Trikuspidalklappenrückflusses (moderate und schwere Ausprägung: 9,4 % vs. 6,4 %, p = 0,006). Im Laufe der Follow-Up-Periode (23,0 Monate ± 3,1 Monate) sind 72 Patienten verstorben. Davon wurde bei vier Patienten eine Herztransplantation durchgeführt. Zwei weitere Patienten mussten aufgrund der vordefinierten Studienkriterien ausgeschlossen werden. Die Mortalität war bei den Patienten der HFrEF signifikant höher im Vergleich zu Erkrankten der HFmrEF-Gruppe (11,4 % vs. 6,4 %, p =0,014). In der multivariaten Cox-Regressionsanalyse konnte die linksventrikuläre Ejektionsfraktion als Determinante der Gesamtkohorte ermittelt werden (Hazard ratio (HR) = 0,607, p = 0,049). Die NYHA-Klasse (HR: 1,990 und 2.041), die Anämie (HR: 1,638 und 1,883), die chronische Niereninsuffizienz (HR: 1,906 und 1,905), die NT-proBNP-Blutspiegel (HR: 2,577 und 2,255) und die moderate bis schwere Ausprägung des Trikuspidalklappenrückflusses (HR: 2,079 und 2,404) stellten nach Adjustierung von Alter, Geschlecht und BMI unabhängige Determinanten auf das Zweijahresüberleben der Gesamtkohorte, als auch der HFrEF-Gruppe dar. Einzig und allein für die neudefinierte HFmrEF-Patientengruppe ließen sich die NT-proBNP-Blutspiegel (HR: 3,873, 95 %, CI: 1,752 – 8,652, p = 0,001) als unabhängige Determinante auf das Zweijahresüberleben ermitteln.
Zusammenfassung:
Die hier vorliegende Studie beschäftigt sich mit den klinischen und echokardiografischen Unterschieden der neudefinierten HFmrEF- zur HFrEF-Patientengruppe auf. Linksventrikuläre Ejektionsfraktion (LVEF), NYHA-Klasse, Anämie, Niereninsuffizienz, NT-proBNP, moderate bis schwere Trikuspidalklappeninsuffizienz stellten Determinanten auf das Zweijahresüberleben der Gesamtkohorte als auch der HFrEF-Patienten. Bei der neudefinierten HFmrEF-Gruppe zeigten sich einzig und allein die erhöhten NTproBNP-Serumspiegel als unabhängige Determinante auf das Zweijahresüberleben der Erkrankten.
Hintergrund
Während eine hochgradige Aortenklappenstenose in vielen Fälle eine
Indikation zum Klappenersatz darstellt, wird bei mittelgradiger
Aortenklappenstenose lediglich eine engmaschige echokardiographische
Kontrolle empfohlen. Es besteht schließlich weitgehender Konsens, dass
diese von Patienten ohne Pumpfunktionsstörung gut kompensiert werden
kann. Unklar bleibt jedoch, ob dies ebenso für Patienten mit LV-Dysfunktion
gilt oder ob diese als eigenständige Kohorte behandelt werden sollten.
Methoden
Insgesamt wurden 67 Patienten des Aortenklappenstenose-Registers im
Universitätsklinikum Würzburg im mittelgradigen Stadium (AÖF 1,1 –
1,5 cm2) mit Erstdiagnose zwischen Mai 2005 und August 2013 und
begleitender systolischer Herzinsuffienz (Mittelwert: Alter 75±8 Jahre, 69%
männlich, EF 38,7±7,7%) retrospektiv analysiert. Diese wurden mit 139
Herzinsuffizienz-Patienten ohne Stenose des DZHI Würzburgs, welche auf
die Parameter Alter, Geschlecht und Ejektionsfraktion gematcht waren,
verglichen (Mittelwert: Alter 74±6 Jahre, 66% männlich, EF 38,9±2,6%).
Ausgeschlossen wurden Patienten mit bikuspider Aortenklappe, Patienten
nach Aortenklappenersatz, Patienten mit anderen höhergradigen Vitien
sowie Patienten mit seltenen, hereditären Kardiomyopathien. Es wurden die
Endpunkte allgemeiner Tod, kardialer Tod und Hospitalisierung wegen
Herzinsuffizienz über einen Beobachtungszeitraum von 3 Jahren untersucht.
Ergebnisse
Während im Kollektiv mit mittelgradiger Aortenklappenstenose (MAKS-Kollektiv)
Hypertonus und Diabetes signifikant häufiger auftraten, hatten im
Vergleichskollektiv deutlich mehr Patienten eine positive Raucheranamnese
und einen Herzinfarkt durchgemacht. Waren im MAKS-Kollektiv Patienten
aus allen NYHA-Klassen gleichmäßig vertreten, so waren im
Vergleichskollektiv vor allem Patienten der NYHA-Klasse II und III
repräsentiert. Hinsichtlich echokardiographischer Messwerte zeigten MAKS-Patienten
zum Baseline-Zeitpunkt in den diastolischen Parametern E/E’-
Verhältnis (18,6±7 vs. 13,7±11, p=0,01) und Dezelerationszeit (232±105 ms
vs. 197±79 ms, p=0,025) schlechtere Werte und hatten häufiger
Vorhofflimmern (37% vs. 22%, p=0,023).
Im Beobachtungszeitraum von 3 Jahren starben 25 (37%) im MAKSKollektiv
vs. 36 (26%) Patienten im Vergleichskollektiv (p=0,075) an
allgemeinen Todesursachen sowie 14 (21%) vs. 15 (11%) Patienten an
kardiovaskulären Ursachen (p=0,035) wohingegen 17 (25%) vs. 43 (31%)
Patienten wegen Herzinsuffizienz hospitalisiert wurden (p=0,57). Im
Stenose-Kollektiv wurden 4 Klappenersatz-Operationen durchgeführt.
In der Cox-Regression zeigte sich das Alter als derart starker Prädiktor, dass
nach Adjustierung auf Alter und Geschlecht der Einfluss der mittelgradigen
Aortenklappenstenose hinsichtlich allgemeinem Tod [HR 1,59 (0,94-2,68),
p=0,085] und kardiovaskulärem Tod [HR 1,73 (0,81-3,68), p=0,157] das
Signifikanzniveau nicht erreichte.
Schlussfolgerung
Zusammenfassend lässt sich anhand dieser Daten sagen, dass Patienten
mit mittelgradiger Aortenklappenstenose und gleichzeitig bestehender LV-Dysfunktion
ein tendenziell schlechteres Outcome im Vergleich zu Patienten
ohne Aortenklappenstenose haben, wohingegen sich ihre Hospitalisierungsrate
nicht unterscheidet.
Die vorliegende Studie sollte die Erwartungshaltung, Akzeptanz sowie die physische und psychische Durchführbarkeit hinsichtlich einer Übergewichts-Simulation bei Kindern und Jugendlichen überprüfen. Die Teilnahme an der Simulation ermöglichte es den Proband*innen, typische Alltagsprobleme adipöser Menschen realitätsnah zu erfahren.
Insgesamt 58 Schülerinnen und Schüler im Alter von 13 bis 16 Jahren nahmen an dem Projekt teil und durchliefen die Übergewichts-Simulation.
Die Ergebnisse zeigten eine positive Erwartungshaltung gegenüber der Simulation. Zudem wurde verdeutlicht, dass der Kurs von der überwiegenden Mehrheit als positiv empfunden und somit akzeptiert wurde. Auch zeigte sich, dass die Übungen physisch und psychisch gut durchführbar waren.
Die ETiCS-Studie (Etiology, Titre-Course, and effect on Survival) ist die bisher größte prospektive europäische Studie, die Ursachen und Entstehungsmechanismen kardialer Autoimmunphänomene untersucht. Ziel dieser Dissertation war die umfassende Charakterisierung der beiden prospektiven ETiCS-Kollektive sowie der Vergleich ihrer demographischen, klinischen, laborchemischen und apparativen Charakteristika zum Zeitpunkt des Studieneinschlusses. Die prospektive ETiCS-Studie umfasste im FAMI-Kollektiv (erster akuter Myokardinfarkt) insgesamt n=180 Patienten und im AMitis-Kollektiv (erste akute Myokarditis) n=96 Patienten. Die demographischen Daten, das kardiovaskuläre Risikoprofil sowie die klinische Symptomatik unserer Patienten entsprachen im Wesentlichen den in der Literatur bereits beschriebenen ähnlichen Vergleichskollektiven, mit dem interessanten Unterschied, dass unsere Infarkt-Patienten deutlich jünger waren (57 ± 8 Jahre), als der Durchschnittspatient mit erstmaligem Myokardinfarkt. Als Schlussfolgerung dieser Arbeit für die klinische Praxis lässt sich durch akribische Erhebung der Anamnese und des kardiovaskulären Risikoprofils eines Patienten mit unklaren kardialen Beschwerden mit einer gewissen Wahrscheinlichkeit ein akuter Myokardinfarkt oder eine akute Myokarditis vorhersagen. Das führende klinische Symptom ist mit Thoraxschmerz und Dyspnoe bei beiden Krankheitsbildern recht ähnlich, jedoch sollte bei führender Belastungsdyspnoe und zeitgleich typischen Nebenkriterien (Fieber, Palpitationen, Infektanamnese) primär an eine Myokarditis gedacht werden. Anhand der Ischämiemarker ist der Ausschluss einer akuten Myokardischämie oder einer akuten Herzmuskelentzündung zwar mit großer Sicherheit möglich, bei erhöhten Werten muss jedoch für eine weitere Differenzierung auch die Klinik, die EKG-Diagnostik und die Echokardiographie mit betrachtet werden. Auch bei nicht eindeutigem EKG-Befund sollte die Indikation zur Koronarangiographie nur in Zusammenschau der genannten Befunde gestellt werden. Sobald sich jedoch der Verdacht auf ein akutes Infarktgeschehen erhärtet, sollte ohne Zeitverzögerung eine invasive Diagnostik erfolgen.
In dieser Studie wurde untersucht, ob die durch Ischämie (45min clamping) induzierte Herunterregulation der Sekretion organischer Anionen, wie postuliert, zu einer verminderten Ausscheidung von PGE2 führt und wie sich die Relation zu Inulin und PAH-Clearance verhält. Es wurde weiterhin untersucht, ob die Medikation mit Nebivolol (2,5mg, 5mg und 10 mg) oder Metoprolol Tartrat (47,5mg) einen positiven Effekt auf das renale Outcome, unter Berücksichtigung des Einflusses auf die PGE2- Ausscheidung, hat.
PGE2 wird im Akuten Nierenversagen vermehrt gebildet und die Sekretion vermindert, dies führt zu einer verstärkten inflammatorischen Situation, aber im Gegenzug auch zu einer verbesserten renalen mikrovaskuläre Blutversorgung.
Es wurde bei SD-Ratten eine renale Ischämie, durch Abklemmen der Arteriae renales, für 45 min operativ herbeigeführt. Die Gabe der Betablocker erfolgte nach 35min Ischämie. Nach 24h wurden Inulin- und PAH-Clearance gemessen. Die Messung der PGE2- Konzentration in Serum und Urin erfolgte mittels kompetitivem ELISA.
Generell ist ein besseres renales Outcome bei den mit Nebivolol und Metoprolol Tartrat behandelten Tieren zu erkennen. Besonders ausgeprägt war dieser Effekt bei der Gruppe Cl-Nb2,5mg-Gruppe zu erkennen. Unter den Clamping-Gruppen war ebenfalls die PGE2-Claerance bei Clamp-Nb2,5mg am höchsten. Eine Korrelation zwischen erhöhter Inulinkonzentration im Urin (besseres outcome) und erhöhter PGE2- Konzentration im Urin konnte nicht nachgewiesen werden.
Anämie (A), Niereninsuffizienz (RI) und Eisenmangel (ID) sind häufige Komorbiditäten der Herzinsuffizienz. Zum ersten Mal wurden in dieser Analyse die Prävalenz sowie der Einfluss auf Mortalität aller drei Komorbiditäten einzeln sowie koinzident in einer Population aus akut dekompensierten Herzinsuffizienzpatienten untersucht. Ebenso fehlten in der Literatur bisher Studien über die Prävalenz und den Einfluss auf die Mortalität von Anämie und Niereninsuffizienz abhängig von den vier AHA/ACC-Stadien bzw. von den verschiedenen Herzinsuffizienztypen HFrEF, HFpEF und dem hinsichtlich Herzinsuffizienz bisher asymptomatischen AHA/ACC-Stadium A/B.
A, RI und ID sind häufig und treten bei Überlebenden nach Hospitalisierung mit akut dekompensierter HFrEF oft zusammen auf. Patienten mit A und RI mit oder ohne ID haben das höchste Risiko für Mortalität. Die Definition und prognostische Rolle des ID nach akuter kardialer Dekompensation erfordert weitere Forschungsbemühungen.
Die Prävalenz von A und insbesondere von RI ist bereits in den aymptomatischen AHA / ACC-Stadien A und B hoch und nimmt mit dem Schweregrad der Herzinsuffizienz zu. Sowohl A als auch RI haben einen individuellen und kumulativen prognostischen Einfluss über das gesamte AHA / ACC-Spektrum.
A und RI waren bei allen Herzinsuffizienztypen häufig. Mehr als 20% der asymptomatischen AHA / ACC-Patienten im Stadium A und B hatten bereits RI. A und RI zeigten einen negativen individuellen und kumulativen prognostischen Einfluss bei allen Herzinsuffizienztypen, einschließlich der asymptomatischen Patienten (bei HFpEF gab es nur einen Trend, höchstwahrscheinlich aufgrund der geringeren Patientenzahl).
Bei Bestehen von A, RI oder ID ist eine sorgfältige Ursachenforschung indiziert im Rahmen eines ganzheitlichen Managements der Herzinsuffizienz mit dem Ziel, die Prognose der Herzinsuffizienz zu verbessern.
Analyse zur Diagnostik und Therapie der kardialen und pulmonalen Sarkoidose. Die Problematik der Diagnose einer Sarkoidose besteht in dem Fehlen eines einzelnen diagnostischen Bestätigungstests. Einem internationalen Konsens entsprechend sind ein kompatibles klinisches und radiologisches Bild, der histopathologische Nachweis von nicht-verkäsenden Granulomen und der Ausschluss von Differentialdiagnosen für die Diagnose einer pulmonalen Sarkoidose entscheidend. In der Diagnostik der kardialen Sarkoidose gibt es keinen Referenzstandard oder international akzeptierten Standard. Trotz widersprüchlicher Datenlage und teils fehlender Wirkungsnachweise ist die immunsuppressive Therapie, in erster Linie mit Kortikosteroiden, die Standardtherapie sowohl der pulmonalen wie auch der kardialen Sarkoidose.
Eine fundierte Ausbildung ist in der interventionellen Kardiologie essentiell, um die teilweise komplexen Prozeduren erfolgreich und sicher durchführen zu können. Bei der perkutanen Koronarintervention (PCI) können u.a. Fehler beim Handling des Führungsdrahtes auftreten. So kann es einerseits zum Drahtverlust, andererseits zur distalen Koronargefäßperforation kommen. Daher ist es sinnvoll, die Technik des Katheterwechsels ohne inadäquate Drahtbewegung vor der ersten Intervention im Herzkatheterlabor am Modell zu trainieren. Für diesen Zweck wurde der DACH-BOSS-Simulator entwickelt, an dem der Katheterwechsel trainiert werden kann.
Die Validität des Modells wurde im Rahmen einer Studie bei 10 Medizinstudenten (S) sowie 10 angehenden interventionellen Kardiologen (F) untersucht. Jeder Teilnehmer führte eine Trainingsreihe bestehend aus 25 Prozeduren durch. Um den Trainingseffekt zu ermitteln, wurden die mittleren Punktzahlen der ersten 3 und der letzten 3 Prozeduren jedes Probanden in der Studenten- und Fortgeschrittenengruppe verglichen. Zur Bestimmung der Konstruktvalidität führte eine dritte Gruppe von 5 Experten (E, > 1000 PCIs) jeweils 3 Prozeduren durch. Ausmaß der Drahtbewegung und benötigte Zeit wurden mit Punkten bewertet und als Skills score dargestellt.
Bei den ersten 3 Prozeduren erzielten die Experten signifikant höhere Werte als die Studenten oder die Fortgeschrittenengruppe (E: 12,9±1,0; S: 7,1±2,6, p = 0,001;
F: 8,3±2,0; p = 0,001; Mann-Whitney-U). Anfänger und Fortgeschrittene durchliefen während der 25 Trainingsprozeduren eine Lernkurve; im Mittel verbesserte sich die Studentengruppe von 7,1±2,6 auf 12,2±2 (p=0,007, Wilcoxon) und die Fortgeschrittenengruppe von 8,3±2,0 auf 13,2±1,0 (p = 0,005, Wilcoxon).
Der DACH-BOSS-Simulator stellt somit ein valides Modell zum Training des Katheterwechsels ohne inadäquate Drahtbewegung dar. Angehende interventionelle Kardiologen können diesen wichtigen Schritt der Prozedur am Modell trainieren und erlernen. Ob die am Simulator erworbenen Fähigkeiten auf die klinische Prozedur übertragbar sind, muss in weiteren Studien untersucht werden.
In der vorliegenden Dissertation wurde anhand von post- hoc Analysen aus dem Datensatz des Interdisziplinären Netzwerkes Herzinsuffizienz (INH, Unique identifier: ISRCTN 23325295) die prognostische Bedeutung depressiver Symptome bei Patienten mit systolischer Herzinsuffizienz bestimmt. Dazu wurden n=852 Patienten untersucht, die zur Baseline alle einen PHQ-9 Fragebogen zur Erhebung ihrer depressiven Symptome ausgefüllt hatten. Es konnte gezeigt werden, dass sich die kürzere Version des PHQ-9, der PHQ-2, ebenso gut zum Screening für Depression eignete und auch ein prognostischer Marker für Tod jeder Ursache und Rehospitalisierung nach 540 Tagen war. Ein Dosis- Wirkungseffekt konnte für zunehmende depressive Symptome nachgewiesen werden. Der PHQ-9 eignete sich als Risikomarker für beide Geschlechter. Es zeigten sich signifikante Unterschiede in den Baseline Charakteristiken und dem depressiven Symptomprofil von Frauen und Männern. Die weiblichen Teilnehmerinnen hatten zusätzlich eine signifikant schlechtere Lebensqualität anhand des krankheitsspezifischen Kansas City Cardiomyopathy Questionnaires. Dafür hatten nur Männer mit vermehrten depressiven Symptomen auch ein erhöhtes Rehospitalisierungsrisiko. Depressive Symptome verschlechterten die Lebensqualität bei beiden Geschlechtern. Die Ergebnisse tragen dazu bei die Aufmerksamkeit für die häufig auftretenden und zu selten diagnostizierten depressiven Symptome bei Herzinsuffizienz zu erhöhen. Der PHQ-2 ist zudem weniger zeitintensiv und kann mündlich erfragt werden. Die Informationen aus den hier gezeigten Geschlechtsunterschieden könnten darüber hinaus in der Zukunft für individuellere Behandlungsziele und Unterstützungsangebote verwendet werden.
Der primäre Hyperaldosteronismus stellt mit einer Prävalenz von 5 - 12% die häufigste endokrine Ursache einer sekundären Hypertonie da. Der Aldosteronexzess beruht in den meisten Fällen auf einer bilateralen Nebennierenhyperplasie, oder einem unilateralen Aldosteron-produzieren Adenom. Die Unterscheidung dieser beiden Entitäten ist zur weiteren Therapieplanung essentiell: Während ein unilaterales Aldosteron-produzierendes Adenom operativ entfernt wird, erhalten Patienten mit bilateraler Nebennierenhyperplasie eine lebenslange medikamentöse Therapie mit Mineralocorticoidantagonisten. Der selektive Nebennierenvenenkatheter stellt den aktuellen Referenzstandard zur Subtypendifferenzierung bei Patienten mit primärem Hyperaldosteronismus dar. Dieser ist allerdings mit zahlreichen Nachteilen, wie beispielsweise hoher Untersucherabhängigkeit und fehlender Standardisierung, behaftet. Die Strahlenbelastung des Nebennierenvenenkatheters wurde bis dato nicht systematisch analysiert.
Im ersten Teil dieser Arbeit wurde daher im Rahmen einer retrospektiven Studie die Strahlenbelastung des Nebennierenvenenkatheters (n=656) an vier deutschen Zentren zwischen 1999 und 2017 untersucht. Primäre Endpunkte waren das Dosisflächenprodukt, die Durchleuchtungszeit und die kalkulierte effektive Dosis. Das mediane Dosisflächenprodukt lag bei 32,5 Gy*cm², die Durchleuchtungszeit bei 18 min, die berechnete effektive Dosis bei 6,4 mSv. Die Strahlenbelastung zeigte allerdings deutliche interinstitutionelle Unterschiede. Hauptursachen hierfür lagen in zentrumsabhängigen Unterschieden in den Untersuchungsmodalitäten, insbesondere der Anzahl an Entnahmestellen und der Verwendung von digitalen Subtraktionsangiographien.
Der zweite Teil der Arbeit widmet sich der Evaluation des Chemokinrezeptors CXCR4 als neues Target in der Differentialdiagnostik des primären Hyperaldosteronismus. Mittels quantitativer real-time PCR und Immunohistochemie konnte eine hohe Expression von CXCR4 in den äußeren Schichten der Nebennierenrinde, der anhängenden Nebenniere benigner Nebennierentumore, sowie in Aldosteron-produzierenden Adenomen beobachtet werden. Ebenso zeigte sich eine Korrelation der CXCR4-Expression mit der Expression von CYP11B2 (Aldosteron-Synthase). Dem entgegengesetzt, war eine fehlende bzw. signifikant niedrige CXCR4-Expression in endokrin inaktiven Nebennierenadenomen nachweisbar. In autoradiographischen Bindungsstudien zeigte der CXCR4-spezifische PET-Tracer 68Ga-Pentixafor eine deutliche und spezifische Bindung an Kryogewebe von Aldosteron-produzierenden Adenomen. Im Anschluss wurden 9 Patienten mit primärem Hyperaldosteronismus mittels 68Ga-Pentixafor-PET/CT untersucht. Der Tracer-Uptake war signifikant höher in Aldosteron-produzierenden Adenomen im Vergleich zu Nebennieren von Patienten, die aufgrund anderer Grunderkrankungen eine 68Ga-Pentixafor-Bildgebung erhalten hatten. Somit stellt eine CXCR4-spezifische Bildgebung eine vielversprechende Möglichkeit zur nicht-invasiven Charakterisierung von Nebennierenraumforderungen bei primärem Hyperaldosteronismus dar.
Abstrakt
Hypertrophe Kardiomyopathie (HCM) ist eine genetisch bedingte Herzmuskelerkrankung mit einer Prävalenz von 0,2 bis 0,6% und einem SCD-Risiko von 0,5 bis 1% pro Jahr. HCM ist die häufigste Ursache für plötzlichen Herztod in jungem Alter. Seit Jahrzehnten wird bei HCM der optimale Vorhersagescore für SCD untersucht. Der erste validierte SCD-Sore bei HCM wurde im Jahr 2014 in die ESC-Leitlinien integriert.
Ziel der Studie:
Vergleich des berechneten SCD-Scores bei HCM aus dem Jahr 2014 vor und nach Alkohol- Septum-Ablation (PTSMA) mit dem erreichten Endpunkt (SCD).
Methoden:
56 Patienten mit hypertropher obstruktiver Kardiomyopathie (HOCM) und Erst-PTSMA im Jahr 2009 wurden eingeschlossen: Alter 53,9 ± 11,5 Jahre, 7% positive Familienanamnese für SCD, 9% ungeklärte Synkope im letzten Jahr, 43% NSVT, maximale LV-Wanddicke 20,2 ± 4,3 mm, maximaler LVOT-Gradient 118 ± 42 mmHg, LA-Durchmesser 45,3 ± 6,4 mm.
Ergebnisse:
Vor dem ersten PTSMA hatten die HOCM-Patienten einen errechneten SCD-Wert von 4,2 ± 3,2%, nach PTSMA von 3,2 ± 2,2%. Wir beobachteten 2 (3,6%) SCD-Fälle in 5 Jahren.
Die PTSMA führte zu einer signifikanten Reduktion des errechneten SCD-Scores von 1,0 ± 2,8%, p <0,05. Diese Reduktion war hauptsächlich durch die signifikante Reduktion des LVOT-Gradienten (durchschnittlich 54 ± 42 mmHg, p <0,05) zurückzuführen.
Fazit:
PTSMA ist eine etablierte Behandlung zur Verbesserung der Symptome bei HOCM-Patienten ohne Hinweis auf eine höhere Mortalität nach induziertem Infarkt. Der ESC-SCD-Score ist nur ein Hilfsalgorithmus für die individuelle Entscheidung bezüglich einer primärprophylaktischen AICD-Implantation.
Morbus Fabry ist eine X-chromosomale rezessiv vererbbare Krankheit, welche zu den lysosomalen Speicherkrankheiten gehört. Aufgrund der niedrigen Inzidenz, des unterschiedlich starken Befalls verschiedener Organe und der sehr breiten Symptomvielfalt, ist sowohl die Diagnosestellung als auch die Therapie schwierig.
Der FASTEX teilt als erstes Scoring-System den Verlauf bei Morbus Fabry als stabil oder instabil ein. Aktuell werden der Morbus Fabry und das Ansprechen einer Therapie klinisch anhand von Änderungen des Lyso-GB3s überwacht.
Diese Dissertation vergleicht retrospektiv den nach FASTEX berechneten Krankheitsverlauf mit dem klinischen Krankheitsverlauf an 104 Patienten mit Morbus Fabry. Ein Anstieg des Lyso-GB3s ≥ 20 % ist hierbei als klinisch instabiler Verlauf definiert.
Der FASTEX teilt 86,4 % der Patienten richtig als stabil ein (≙ eines Anstiegs des Lyso-GB3s < 20 %). Im Gegensatz dazu wird nur jeder 10. Patient mit einem klinisch instabilen Krankheitsverlauf von dem FASTEX korrekt als instabil erfasst. Diese Ergebnisse unterscheiden sich von bisherigen Studien über den FASTEX.
Einige der untersuchten Patienten hatten genau einen FASTEX-Score von 20 % und weisen somit gemäß der Definition einen nach FASTEX berechneten instabilen Krankheitsverlauf (≙ FASTEX-Score ≥ 20 %) auf. Jedoch war der Großteil dieser Patienten klinisch stabil. In zukünftigen Studien ist daher zu klären, ob der Grenzwert bei dem FASTEX zwischen einem stabilen und instabilen Krankheitsverlauf angepasst werden muss und ob das Lyso-GB3 nicht in die Berechnung des FASTEX einbezogen werden sollte.
In dieser Arbeit wurden die Signalwege an Myokard, Leber und Skelettmuskulatur untersucht, die den Veränderungen des Glucosestoffwechsels im zeitlichen Verlauf bei TAC-induzierter systolischer Herzinsuffizienz zu Grunde liegen können.
Es wurde dafür über eine TAC-Operation am Mausmodell eine Herzinsuffizienz induziert. Erniedrigte Nüchtern-Blutzuckerwerte und ein erhöhter myokardialer Glucoseverbrauch fanden sich als die wesentlichen metabolischen Veränderungen in diesem Mausmodell mit TAC-induzierter Herzinsuffizienz. In Vorarbeiten ergaben die GTTs nach zwei Wochen eine erhöhte Glucoseaufnahme nach TAC, die nicht durch eine verstärkte Insulinsignaltransduktion hervorgerufen wurde, da sich die Insulinkonzentrationen und die Insulinsensitivität zwischen den Gruppen nicht unterschieden. Der Nüchternblutzuckers war bei den TAC-operierten Tieren niedriger als bei den Sham-operierten. In der PET-Bildgebung wurde eine erhöhte Glucoseaufnahme im Herzen bei den TAC-Tieren verglichen mit den Sham-operierten Tieren gezeigt.
Im Rahmen dieser Arbeit wurden nun nach einem bzw. sechs Monaten bei den Versuchsgruppen Western Blot Analysen sowie Glykogenassays durchgeführt. Zudem wurde untersucht, wie sich eine zusätzliche Sucrosefütterung über sechs Monate ab OP auswirkt.
Dabei fand sich eine erhöhte Expression des basalen Glucosetransporters GLUT1 am Myokard, passend zur erhöhten basalen Glucoseaufnahme im PET. Die Expression des insulinabhängigen Transporters GLUT4 hingegen war - wie bereits in anderen Arbeiten zu Hypertrophie und Herzinsuffizienz - vermindert.
Der hepatische und muskuläre Glykogengehalts war nach TAC reduziert, wobei die Depletion der hepatischen Speicher bereits nach einem Monat, im Muskel erst nach sechs Monaten signifikant war.
Die Regulation der muskulären Glykogenreserven wurde in unserem Modell möglicherweise über Akt und Hexokinase II vermittelt. Einen Monat nach TAC war am Skelettmuskel eine vorübergehende Reduktion der Akt-Phosphorylierung zu verzeichnen, was über Aktivierung der Glykogensynthase zur Aufrechterhaltung der muskulären Glykogenspeicher beitragen kann. Sechs Monate nach TAC-OP war die Akt-Expression im Skelettmuskel dann jedoch verstärkt, was zwar einen Kompensationsmechanismus zur Vermeidung einer Muskel-Atrophie darstellt, jedoch durch Inaktivierung der Glykogensynthase sinkende Glykogenreserven bedingt. Gleichzeitig war eine Steigerung der Expression von Hexokinase II zu beobachten, die durch Bildung von Glucose-6-Phosphat die intrazellluläre Glucoseverfügbarkeit kompensiert.
Bei zusätzlicher Sucrosefütterung über sechs Monate zeigte sich im Muskel der TAC-operierten Tiere keine kompensatorische Steigerung von Akt- und HKII-Expression mehr, und auch die Entleerung der muskulären Glykogenspeicher war vollständig abgefangen. Ebenso wurde die Entleerung der hepatischen Glykogenvorräte durch Sucrose-Substitution verhindert, was für eine ausreichende Glucoseverfügbarkeit spricht und die Normalisierung der Glucosetoleranz erklärt. Wichtige Regulatoren der Leber-Glykogensynthese, z.B. GLUT2 und HK IV, oder der zellulären Energie-Homöostase, z.B. AMPK, waren hingegen zu keinem Zeitpunkt wesentlich verändert.
Zusammenfassend konnten somit durch eine Sucrosesubstitution die bei TAC-induzierter Herzinsuffizienz reduzierten Glykogenspeicher in Muskel und Leber erhalten, der systemische Glucosestoffwechsel normalisiert und eine Reduktion der Mortalität erzielt werden. Insgesamt lässt sich feststellen, dass sich Strategien, die zu einer Wiederherstellung der systemischen Glucoseverfügbarkeit beitragen, möglicherweise positiv auf eine Herzinsuffizienztherapie auswirken könnten.
Aims Acute myocardial infarction (MI) is the major cause of chronic heart failure. The activity of blood coagulation factor XIII (FXIIIa) plays an important role in rodents as a healing factor after MI, whereas its role in healing and remodelling processes in humans remains unclear. We prospectively evaluated the relevance of FXIIIa after acute MI as a potential early prognostic marker for adequate healing.
Methods and results This monocentric prospective cohort study investigated cardiac remodelling in patients with ST-elevation MI and followed them up for 1 year. Serum FXIIIa was serially assessed during the first 9 days after MI and after 2, 6, and 12 months. Cardiac magnetic resonance imaging was performed within 4 days after MI (Scan 1), after 7 to 9 days (Scan 2), and after 12 months (Scan 3). The FXIII valine-to-leucine (V34L) single-nucleotide polymorphism rs5985 was genotyped. One hundred forty-six patients were investigated (mean age 58 ± 11 years, 13% women). Median FXIIIa was 118 % (quartiles, 102–132%) and dropped to a trough on the second day after MI: 109%(98–109%; P < 0.001). FXIIIa recovered slowly over time, reaching the baseline level after 2 to 6 months and surpassed baseline levels only after 12 months: 124 % (110–142%). The development of FXIIIa after MI was independent of the genotype. FXIIIa on Day 2 was strongly and inversely associated with the relative size of MI in Scan 1 (Spearman’s ρ = –0.31; P = 0.01) and Scan 3 (ρ = –0.39; P < 0.01) and positively associated with left ventricular ejection fraction: ρ = 0.32 (P < 0.01) and ρ = 0.24 (P = 0.04), respectively.
Conclusions FXIII activity after MI is highly dynamic, exhibiting a significant decline in the early healing period, with reconstitution 6 months later. Depressed FXIIIa early after MI predicted a greater size of MI and lower left ventricular ejection fraction after 1 year. The clinical relevance of these findings awaits to be tested in a randomized trial.
Der primäre Hyperaldosteronismus ist eine der häufigsten Ursachen einer sekundären Hypertonie. Entscheidend für eine adäquate Therapie ist die Subtypen-Differenzierung zwischen Aldosteron produzierenden Adenomen und bilateralen Hyperplasien der Nebennierenrinde. Hierfür wird heutzutage eine selektive Katheterisierung der Nebennierenvenen durchgeführt. Diese Untersuchung ist jedoch sehr aufwendig und geht mit einer hohen Strahlenbelastung einher. Daher wurden im Rahmen dieser Arbeit 11 fluorierte Inhibitoren der Aldosteronsynthase (CYP11B2) auf ihre in-vitro- und in-vivo-Wirksamkeit und Selektivität untersucht. Ziel war es, über eine gezielte Bindung radiofluorierter Tracer an Aldosteron produzierenden Zellen eine nicht invasive Bildgebung zur Subtypendifferenzierung beim primären Hyperaldosteronismus zu entwickeln. Hierbei konnten mehrere vielversprechende Substanzen gefunden werden, die selektiv an CYP11B2 binden und die Aldosteronproduktion hemmen. Diese können somit möglicherweise für die Subtypendifferenzierung beim primären Hyperaldosteronismus eingesetzt werden.
Background
Fabry disease (FD) is an X‐linked lysosomal storage and multi‐system disorder due to mutations in the α‐galactosidase A (α‐GalA) gene. We investigated the impact of individual amino acid exchanges in the α‐GalA 3D‐structure on the clinical phenotype of FD patients.
Patients and methods
We enrolled 80 adult FD patients with α‐GalA missense mutations and stratified them into three groups based on the amino acid exchange location in the α‐GalA 3D‐structure: patients with active site mutations, buried mutations and other mutations. Patient subgroups were deep phenotyped for clinical and laboratory parameters and FD‐specific treatment.
Results
Patients with active site or buried mutations showed a severe phenotype with multi‐organ involvement and early disease manifestation. Patients with other mutations had a milder phenotype with less organ impairment and later disease onset. α‐GalA activity was lower in patients with active site or buried mutations than in those with other mutations (P < 0.01 in men; P < 0.05 in women) whilst lyso‐Gb3 levels were higher (P < 0.01 in men; <0.05 in women).
Conclusions
The type of amino acid exchange location in the α‐GalA 3D‐structure determines disease severity and temporal course of symptom onset. Patient stratification using this parameter may become a useful tool in the management of FD patients.
Ischemia/reperfusion injury is a major cause of acute kidney injury (AKI). AKI is characterized by a sudden decrease in kidney function, systemic inflammation, oxidative stress, and dysregulation of the sodium, potassium, and water channels. While AKI leads to uremic encephalopathy, epidemiological studies have shown that AKI is associated with a subsequent risk for developing stroke and dementia. To get more insights into kidney–brain crosstalk, we have created an in vitro co-culture model based on human kidney cells of the proximal tubule (HK-2) and brain microvascular endothelial cells (BMEC). The HK-2 cell line was grown to confluence on 6-well plates and exposed to oxygen/glucose deprivation (OGD) for 4 h. Control HK-2 cells were grown under normal conditions. The BMEC cell line cerebED was grown to confluence on transwells with 0.4 μm pores. The transwell filters seeded and grown to confluence with cereEND were inserted into the plates with HK-2 cells with or without OGD treatment. In addition, cerebEND were left untreated or treated with uremic toxins, indole-3-acetic acid (IAA) and indoxyl sulfate (IS). The protein and mRNA expression of selected BBB-typical influx transporters, efflux transporters, cellular receptors, and tight junction proteins was measured in BMECs. To validate this in vitro model of kidney–brain interaction, we isolated brain capillaries from mice exposed to bilateral renal ischemia (30 min)/reperfusion injury (24 h) and measured mRNA and protein expression as described above. Both in vitro and in vivo systems showed similar changes in the expression of drug transporters, cellular receptors, and tight junction proteins. Efflux pumps, in particular Abcb1b, Abcc1, and Abcg2, have shown increased expression in our model. Thus, our in vitro co-culture system can be used to study the cellular mechanism of kidney and brain crosstalk in renal ischemia/reperfusion injury.
In Eurotransplant kidney allocation system (ETKAS), candidates can be considered unlimitedly for repeated re‐transplantation. Data on outcome and benefit are indeterminate. We performed a retrospective 15‐year patient and graft outcome data analysis from 1464 recipients of a third or fourth or higher sequential deceased donor renal transplantation (DDRT) from 42 transplant centers. Repeated re‐DDRT recipients were younger (mean 43.0 vs. 50.2 years) compared to first DDRT recipients. They received grafts with more favorable HLA matches (89.0% vs. 84.5%) but thereby no statistically significant improvement of patient and graft outcome was found as comparatively demonstrated in 1st DDRT. In the multivariate modeling accounting for confounding factors, mortality and graft loss after 3rd and ≥4th DDRT (P < 0.001 each) and death with functioning graft (DwFG) after 3rd DDRT (P = 0.001) were higher as compared to 1st DDRT. The incidence of primary nonfunction (PNF) was also significantly higher in re‐DDRT (12.7%) than in 1st DDRT (7.1%; P < 0.001). Facing organ shortage, increasing waiting time, and considerable mortality on dialysis, we question the current policy of repeated re‐DDRT. The data from this survey propose better HLA matching in first DDRT and second DDRT and careful selection of candidates, especially for ≥4th DDRT.
Background
Adrenocortical carcinoma (ACC) is a rare endocrine malignancy. Tumor-related glucocorticoid excess is present in similar to 60% of patients and associated with particularly poor prognosis. Results of first clinical trials using immune checkpoint inhibitors were heterogeneous. Here we characterize tumor-infiltrating T lymphocytes (TILs) in ACC in association with glucocorticoids as potential explanation for resistance to immunotherapy.
Methods
We performed immunofluorescence analysis to visualize tumor-infiltrating T cells (CD3\(^+\)), T helper cells (CD3\(^+\)CD4\(^+\)), cytotoxic T cells (CD3\(^+\)CD8\(^+\)) and regulatory T cells (Tregs; CD3\(^+\)CD4\(^+\)FoxP3\(^+\)) in 146 ACC tissue specimens (107 primary tumors, 16 local recurrences, 23 metastases). Quantitative data of immune cell infiltration were correlated with clinical data (including glucocorticoid excess).
Results
86.3% of ACC specimens showed tumor infiltrating T cells (7.7 cells/high power field (HPF)), including T helper (74.0%, 6.7 cells/HPF), cytotoxic T cells (84.3%, 5.7 cells/HPF) and Tregs (49.3%, 0.8 cells/HPF). The number of TILs was associated with better overall survival (HR for death: 0.47, 95% CI 0.25 to 0.87), which was true for CD4\(^+\)- and CD8\(^+\) subpopulations as well. In localized, non-metastatic ACC, the favorable impact of TILs on overall and recurrence-free survival was manifested even independently of ENSAT (European Network for the Study of Adrenal Tumors) stage, resection status and Ki67 index. T helper cells were negatively correlated with glucocorticoid excess (Phi=-0.290, p=0.009). Patients with glucocorticoid excess and low TILs had a particularly poor overall survival (27 vs. 121 months in patients with TILs without glucocorticoid excess).
Conclusion
Glucocorticoid excess is associated with T cell depletion and unfavorable prognosis. To reactivate the immune system in ACC by checkpoint inhibitors, an inhibition of adrenal steroidogenesis might be pivotal and should be tested in prospective studies.
Background: Proportions of patients dying from the coronavirus disease-19 (COVID-19) vary between different countries. We report the characteristics; clinical course and outcome of patients requiring intensive care due to COVID-19 induced acute respiratory distress syndrome (ARDS).
Methods: This is a retrospective, observational multicentre study in five German secondary or tertiary care hospitals. All patients consecutively admitted to the intensive care unit (ICU) in any of the participating hospitals between March 12 and May 4, 2020 with a COVID-19 induced ARDS were included.
Results: A total of 106 ICU patients were treated for COVID-19 induced ARDS, whereas severe ARDS was present in the majority of cases. Survival of ICU treatment was 65.0%. Median duration of ICU treatment was 11 days; median duration of mechanical ventilation was 9 days. The majority of ICU treated patients (75.5%) did not receive any antiviral or anti-inflammatory therapies. Venovenous (vv) ECMO was utilized in 16.3%. ICU triage with population-level decision making was not necessary at any time. Univariate analysis associated older age, diabetes mellitus or a higher SOFA score on admission with non-survival during ICU stay.
Conclusions: A high level of care adhering to standard ARDS treatments lead to a good outcome in critically ill COVID-19 patients.
Objectives
The severity of Coronavirus Disease 2019 (COVID-19) is largely determined by the immune response. First studies indicate altered lymphocyte counts and function. However, interactions of pro- and anti-inflammatory mechanisms remain elusive. In the current study we characterized the immune responses in patients suffering from severe COVID-19-induced acute respiratory distress syndrome (ARDS).
Methods
This was a single-center retrospective study in patients admitted to the intensive care unit (ICU) with confirmed COVID-19 between March 14th and May 28th 2020 (n = 39). Longitudinal data were collected within routine clinical care, including flow-cytometry of lymphocyte subsets, cytokine analysis and growth differentiation factor 15 (GDF-15). Antibody responses against the receptor binding domain (RBD) of Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) Spike protein were analyzed.
Results
All patients suffered from severe ARDS, 30.8% died. Interleukin (IL)-6 was massively elevated at every time-point. The anti-inflammatory cytokine IL-10 was concomitantly upregulated with IL-6. The cellular response was characterized by lymphocytopenia with low counts of CD8+ T cells, natural killer (NK) and naïve T helper cells. CD8+ T and NK cells recovered after 8 to 14 days. The B cell system was largely unimpeded. This coincided with a slight increase in anti-SARS-CoV-2-Spike-RBD immunoglobulin (Ig) G and a decrease in anti-SARS-CoV-2-Spike-RBD IgM. GDF-15 levels were elevated throughout ICU treatment.
Conclusions
Massively elevated levels of IL-6 and a delayed cytotoxic immune defense characterized severe COVID-19-induced ARDS. The B cell response and antibody production were largely unimpeded. No obvious imbalance of pro- and anti-inflammatory mechanisms was observed, with elevated GDF-15 levels suggesting increased tissue resilience.
Mitotane is the only approved drug for advanced adrenocortical carcinoma (ACC) and no biomarkers are available to predict attainment of therapeutic plasma concentrations and clinical response. Aim of the study was to evaluate the suitability of cytochrome P450(CYP)2W1 and CYP2B6 single nucleotide polymorphisms (SNPs) as biomarkers. A multicenter cohort study including 182 ACC patients (F/M = 121/61) treated with mitotane monotherapy after radical resection (group A, n = 103) or in not completely resectable, recurrent or advanced disease (group B, n = 79) was performed. CYP2W1*2, CYP2W1*6, CYP2B6*6 and CYP2B6 rs4803419 were genotyped in germline DNA. Mitotane blood levels were measured regularly. Response to therapy was evaluated as time to progression (TTP) and disease control rate (DCR). Among investigated SNPs, CYP2W1*6 and CYP2B6*6 correlated with mitotane treatment only in group B. Patients with CYP2W1*6 (n = 21) achieved less frequently therapeutic mitotane levels (>14 mg/L) than those with wild type (WT) allele (76.2% vs 51.7%, p = 0.051) and experienced shorter TTP (HR = 2.10, p = 0.019) and lower DCR (chi-square = 6.948, p = 0.008). By contrast, 55% of patients with CYP2B6*6 vs. 28.2% WT (p = 0.016) achieved therapeutic range. Combined, a higher rate of patients with CYP2W1*6WT+CYP2B6*6 (60.6%) achieved mitotane therapeutic range (p = 0.034). In not completely resectable, recurrent or advanced ACC, CYP2W1*6 SNP was associated with a reduced probability to reach mitotane therapeutic range and lower response rates, whereas CYP2B6*6 correlated with higher mitotane levels. The association of these SNPs may predict individual response to mitotane.
Impact of Interparticle Interaction on Thermodynamics of Nano-Channel Transport of Two Species

Understanding the function and control of channel transport is of paramount importance for cell physiology and nanotechnology. In particular, if several species are involved, the mechanisms of selectivity, competition, cooperation, pumping, and its modulation need to be understood. What lacks is a rigorous mathematical approach within the framework of stochastic thermodynamics, which explains the impact of interparticle in-channel interactions on the transport properties of the respective species. To achieve this, stochastic channel transport of two species is considered in a model, which different from mean field approaches, explicitly conserves the spatial correlation of the species within the channel by analysis of the stochastic dynamics within a state space, the elements of which are the channel’s spatial occupation states. The interparticle interactions determine the stochastic transitions between these states. Local flow and entropy production in this state space reveal the respective particle flows through the channel and the intensity of the Brownian ratchet like rectifying forces, which these species exert mutually on each other, together with its thermodynamic effectiveness and costs. Perfect coupling of transport of the two species is realized by an attractive empty channel and strong repulsive forces between particles of the same species. This confines the state space to a subspace with circular topology, in which the concentration gradients as thermodynamic driving forces act in series, and channel flow of both species becomes equivalent. For opposing concentration gradients, this makes the species with the stronger gradient the driving, positive entropy producing one; the other is driven and produces negative entropy. Gradients equal in magnitude make all flows vanish, and thermodynamic equilibrium occurs. A differential interparticle interaction with less repulsive forces within particles of one species but maintenance of this interaction for the other species adds a bypass path to this circular subspace. On this path, which is not involved in coupling of the two species, a leak flow of the species with less repulsive interparticle interaction emerges, which is directed parallel to its concentration gradient and, hence, produces positive entropy here. Different from the situation with perfect coupling, appropriate strong opposing concentration gradients may simultaneously parallelize the flow of their respective species, which makes each species produce positive entropy. The rectifying potential of the species with the bypass option is diminished. This implies the existence of a gradient of the other species, above which its flow and gradient are parallel for any gradient of the less coupled species. The opposite holds for the less coupled species. Its flow may always be rectified and turned anti-parallel to its gradient by a sufficiently strong opposing gradient of the other one.
Long-term treatment effect studies in large female Fabry patient groups are challenging to design because of phenotype heterogeneity and lack of appropriate comparison groups, and have not been reported. We compared long-term cardiomyopathy and kidney function outcomes after agalsidase beta treatment with preceding treatment-naive outcomes.
Methods and results Self-controlled pretreatment and post-treatment comparison (piecewise mixed linear modelling) included Fabry female patients ≥18 years at treatment initiation who received agalsidase beta (0.9–1.1 mg/kg every other week) for ≥2 years, with ≥2 pretreatment and ≥2 post-treatment outcome measurements during 10-year follow-up. Left ventricular posterior wall thickness (LVPWT)/interventricular septal thickness (IVST) and estimated glomerular filtration rate (eGFR, Chronic Kidney Disease Epidemiology Collaboration creatinine equation) analyses included 42 and 86 patients, respectively, aged 50.0 and 46.3 years at treatment initiation, respectively. LVPWT and IVST increased pretreatment (follow-up 3.5 years) but stabilized during 3.6 years of treatment (LVPWT: n = 38, slope difference [95% confidence interval (CI)] = - 0.41 [ - 0.68, - 0.15] mm/year, P\(_{pre–post difference}\)<0.01; IVST: n = 38, slope difference =-0.32 [-0.67, 0.02] mm/year, P\(_{pre–post difference}\) = 0.07). These findings were not modified by renal involvement or antiproteinuric agent use. Compared with the treatment-naive period (follow-up 3.6 years), eGFR decline remained modest and stabilized within normal ranges during 4.1 years of treatment (slope difference, 95% CI: -0.13 [-1.15, 0.89] mL/min/1.73m\(^2\)/year, P\(_{pre–post difference}\) = 0.80).
Conclusions Cardiac hypertrophy, progressing during pretreatment follow-up, appeared to stabilize during sustained agalsidase beta treatment. eGFR decline remained within normal ranges. This suggests that treatment may prevent further Fabry-related progression of cardiomyopathy in female patients and maintain normal kidney function.
This longitudinal study was performed to evaluate the feasibility of detecting the interaction between wall shear stress (WSS) and plaque development. 20 ApoE\(^{-/-}\)mice were separated in 12 mice with Western Diet and 8 mice with Chow Diet. Magnetic resonance (MR) scans at 17.6 Tesla and histological analysis were performed after one week, eight and twelve weeks. Allin vivoMR measurements were acquired using a flow sensitive phase contrast method for determining vectorial flow. Histological sections were stained with Hematoxylin and Eosin, Elastica van Gieson and CD68 staining. Data analysis was performed using Ensight and a Matlab-based "Flow Tool". The body weight of ApoE\(^{-/-}\)mice increased significantly over 12 weeks. WSS values increased in the Western Diet group over the time period; in contrast, in the Chow Diet group the values decreased from the first to the second measurement point. Western Diet mice showed small plaque formations with elastin fragmentations after 8 weeks and big plaque formations after 12 weeks; Chow Diet mice showed a few elastin fragmentations after 8 weeks and small plaque formations after 12 weeks. Favored by high-fat diet, plaque formation results in higher values of WSS. With wall shear stress being a known predictor for atherosclerotic plaque development, ultra highfield MRI can serve as a tool for studying the causes and beginnings of atherosclerosis.
Adrenocortical carcinoma (ACC) is a rare endocrine malignancy with an unfavorable prognosis. Despite the poor prognosis in the majority of patients, no improvements in treatment strategies have been achieved. Therefore, the discovery of new prognostic biomarkers is of enormous interest. Sterol-O-acyl transferase 1 (SOAT1) is involved in cholesterol esterification and lipid droplet formation. Recently, it was demonstrated that SOAT1 inhibition leads to impaired steroidogenesis and cell viability in ACC. To date, no studies have addressed the impact of SOAT1 expression on ACC prognosis and clinical outcomes. We evaluated SOAT1 expression by quantitative real-time polymerase chain reaction and immunohistochemistry in a tissue microarray of 112 ACCs (Weiss score ≥ 3) from adults treated in a single tertiary center in Brazil. Two independent pathologists evaluated the immunohistochemistry results through a semiquantitative approach (0–4). We aimed to evaluate the correlation between SOAT1 expression and clinical, biochemical and anatomopathological parameters, recurrence-free survival (RFS), progression-free survival (PFS), and overall survival (OS). SOAT1 protein expression was heterogeneous in this cohort, 37.5% of the ACCs demonstrated a strong SOAT1 protein expression (score > 2), while 62.5% demonstrated a weak or absent protein expression (score ≤ 2). Strong SOAT1 protein expression correlated with features of high aggressiveness in ACC, such as excessive tumor cortisol secretion (p = 0.01), an advanced disease stage [European Network for the Study of Adrenal Tumors (ENSAT) staging system 3 and 4 (p = 0.011)] and a high Ki67 index (p = 0.002). In multivariate analysis, strong SOAT1 protein expression was an independent predictor of a reduced OS (hazard ratio (HR) 2.15, confidence interval (CI) 95% 1.26–3.66; p = 0.005) in all patients (n = 112), and a reduced RFS (HR 2.1, CI 95% 1.09–4.06; p = 0.027) in patients with localized disease at diagnosis (n = 83). Our findings demonstrated that SOAT1 protein expression has prognostic value in ACC and reinforced the importance of investigating SOAT1 as a possible therapeutic target for patients with ACC.
Context
Primary aldosteronism (PA) is the most frequent form of endocrine hypertension. Besides its deleterious impact on cardiovascular target organ damage, PA is considered to cause osteoporosis.
Patients and methods
We assessed bone turnover in a subset of 36 postmenopausal women with PA. 18 patients had unilateral PA and were treated by adrenalectomy, whereas 18 patients had bilateral PA and received mineralocorticoid receptor antagonist (MRA) therapy respectively. 18 age- and BMI-matched females served as controls. To estimate bone remodeling, we measured the bone turnover markers intact procollagen 1 N-terminal propeptide, bone alkaline phosphatase, osteocalcin and tartrate resistant acid phosphatase 5b in plasma by chemiluminescent immunoassays at time of diagnosis and one year after initiation of treatment.

Results
Compared with controls, patients with PA had mildly elevated osteocalcin at baseline (p = 0.013), while the other bone markers were comparable between both groups. There were no differences between the unilateral and the bilateral PA subgroup. One year after initiation of MRA treatment with spironolactone bone resorption and bone formation markers had significantly decreased in patients with bilateral PA. In contrast, patients adrenalectomized because of unilateral PA showed no significant change of bone turnover markers.
Conclusion
This study shows that aldosterone excess in postmenopausal women with PA is not associated with a relevant increase of bone turnover markers at baseline. However, we observed a significant decrease of bone markers in patients treated with spironolactone, but not in patients treated by adrenalectomy.
We have previously identified serum miR-483-5p as a preoperative diagnosis and prognosis biomarker for adrenocortical cancer (ACC). Here, we aimed to determine whether circulating miR-483-5p levels measured 3 months post-operatively distinguished patients with good prognosis (no recurrence for at least 3 years; NR3yrs) from patients with poor prognosis (recurrence or death within 3 years after surgery; R < 3yrs). We conducted a single-center retrospective analysis using sera from 48 patients with ACC that were initially non-metastatic and treated by surgery. Sera sampled within 3 months after surgery were available in 26 patients. MiR-483-5p absolute circulating levels were measured using quantitative PCR. Thirteen patients showed a recurrence before 3 years (=R < 3yrs). Thirteen patients showed no recurrence within 3 years, including 11 patients with a follow-up longer than 3 years (=NR3yrs). Serum miR-483-5p levels were higher in R < 3yrs than in NR3yrs: 1,541,990 ± 428,377 copies/mL vs. 388,457 ± 62,169 copies/mL (p = 0.002). Receiver operating characteristic analysis showed that a value of 752,898 copies/mL distinguished R < 3yrs from NR3yrs with 61.5% sensitivity (CI 31.6–86.1) and 100% specificity (CI 71.5–100) with an area under the curve of 0.853. Patients with a value below this threshold had a significantly longer recurrence-free and overall survival. In multivariate analysis, miR-483-5p provided the single best prognostic value for recurrence-free survival (RFS) (hazard ratio (HR) for recurrence 5.98, p < 0.011) but not for overall survival. Our study suggests that serum miR-483-5p is a potent early post-operative biomarker for ACC prognosis that might be a better predictor of RFS than currently used markers.
The SF-1 transcription factor target gene FATE1 encodes a cancer-testis antigen that has an important role in regulating apoptosis and response to chemotherapy in adrenocortical carcinoma (ACC) cells. Autoantibodies directed against FATE1 were previously detected in patients with hepatocellular carcinoma. In this study, we investigated the prevalence of circulating anti-FATE1 antibodies in pediatric and adult patients with adrenocortical tumors using three different methods (immunofluorescence, ELISA and Western blot). Our results show that a pervasive anti-FATE1 immune response is present in those patients. Furthermore, FATE1 expression is a robust prognostic indicator in adult patients with ACC and is associated with increased steroidogenic and decreased immune response gene expression. These data can open perspectives for novel strategies in ACC immunotherapy.
Patients affected by gastroenteropancreatic–neuroendocrine tumors (GEP–NETs) have an increased risk of developing osteopenia and osteoporosis, as several factors impact on bone metabolism in these patients. In fact, besides the direct effect of bone metastasis, bone health can be affected by hormone hypersecretion (including serotonin, cortisol, and parathyroid hormone-related protein), specific microRNAs, nutritional status (which in turn could be affected by medical and surgical treatments), and vitamin D deficiency. In patients with multiple endocrine neoplasia type 1 (MEN1), a hereditary syndrome associated with NET occurrence, bone damage may carry other consequences. Osteoporosis may negatively impact on the quality of life of these patients and can increment the cost of medical care since these patients usually live with their disease for a long time. However, recommendations suggesting screening to assess bone health in GEP–NET patients are missing. The aim of this review is to critically analyze evidence on the mechanisms that could have a potential impact on bone health in patients affected by GEP–NET, focusing on vitamin D and its role in GEP–NET, as well as on factors associated with MEN1 that could have an impact on bone homeostasis.
Fabry Disease (FD) is a rare, X-linked, lysosomal storage disease that mainly causes renal, cardiac and cerebral complications. Enzyme replacement therapy (ERT) with recombinant alpha-galactosidase A is available, but approximately 50% of male patients with classical FD develop inhibiting anti-drug antibodies (iADAs) that lead to reduced biochemical responses and an accelerated loss of renal function. Once immunization has occurred, iADAs tend to persist and tolerization is hard to achieve. Here we developed a pre-treatment prediction model for iADA development in FD using existing data from 120 classical male FD patients from three European centers, treated with ERT. We found that nonsense and frameshift mutations in the α-galactosidase A gene (p = 0.05), higher plasma lysoGb3 at baseline (p < 0.001) and agalsidase beta as first treatment (p = 0.006) were significantly associated with iADA development. Prediction performance of a Random Forest model, using multiple variables (AUC-ROC: 0.77) was compared to a logistic regression (LR) model using the three significantly associated variables (AUC-ROC: 0.77). The LR model can be used to determine iADA risk in individual FD patients prior to treatment initiation. This helps to determine in which patients adjusted treatment and/or immunomodulatory regimes may be considered to minimize iADA development risk.
The prevalence of cardiovascular diseases (CVD) increases dramatically with age. Nevertheless, most of the basic research in cardiology has been conducted on young healthy animals which may not necessarily reflect the situation observed in the clinic. The heart undergoes profound changes in elderly, including molecular alterations, myocardial hypertrophy, interstitial fibrosis and functional decline. To date, numerous approaches exist to explain mechanisms of the cardiac aging process whereupon inflammation and immune activity are of increasing interest. Myocardial aging is temporally associated with chronic low-grade systemic inflammation and accumulation of memory T-cells. However, a possible causal relationship between these two phenomena has not yet been investigated. Thus, aim of the present study was to assess how immunological mechanisms contribute to the myocardial aging process.
Herein, the healthy murine heart was found to harbor all major resident leukocyte populations, including macrophages (CD45+CD11b+Ly6G-), granulocytes (CD45+ CD11b+Ly6G+), T-cells (CD45+CD11b-CD3e+), B-cells (CD45+CD11b-B220+) at frequencies that largely surpass those found in skeletal muscles. Age-related structural alterations and functional impairment occur simultaneously with significant shifts of the tissue resident leukocyte composition. Gene expression analyses performed on bulk myocardial samples revealed higher expression levels of TNF and INF- suggesting that in situ inflammation plays a role in the myocardial aging process. Aging was furthermore accompanied by a significant increase in size and cellularity of mediastinal, heart draining lymph nodes (med LN). Moreover, the med LNs harvested from aged mice showed a strong accumulation of effector-memory T-cells (CD44+CD62L-), mainly exhibiting a pro-inflammatory phenotype (Foxp3-, TNF+, IFN- γ+). None of these alterations were observed in popliteal lymph nodes of aged mice, indicating that they might be site-specific.
Next, to go beyond mere associative evidence and examine underlying mechanisms, the myocardial aging process was comprehensively characterized in mice lacking B- (µMT) or CD4+ T-cells (CD4ko). Our analyses revealed that aged CD4+ T-cell-deficient, but not B-cell-deficient mice, exhibit a lower in situ inflammatory tone and preserved ventricular function, as compared to age-matched wild type controls. No differences in the expression levels of genes related to fibrosis were observed in the groups.
Taken together, the results of this study indicate that heart-directed immune responses may spontaneously arise in the elderly, even in the absence of a clear tissue damage or concomitant infection. The T-cell-mediated immunosenescence profile might be particularly associated with age-related myocardial inflammation and functional decline, but not with tissue remodeling. These observations might shed new light on the emerging role of T cells in myocardial diseases, which primarily affect the elderly population.
Die Datenlage zu End-of-Life (EOL)-Entscheidungen im intensivmedizinischen Kontext ist bislang relativ begrenzt. Daher bestand das Ziel dieser Studie darin, die Qualität von Kommunikation, Entscheidungsstrukturen und Sterbeprozessen in der Intensivmedizin beim Wechsel von kurativem zu palliativem Therapieziel aus Sicht von Ärzten und Pflegenden zu erfassen. Dazu wurde ein Fragebogen entwickelt, der neben quantitativen Items auch offene Fragen enthielt, die mittels qualitativer Inhaltsanalyse ausgewertet wurden. Außerdem wurden Hypothesen über berufsgruppenassoziierte Unterschiede in der Bewertung von EOL-Entscheidungen überprüft.
Ende 2014 wurden die Mitarbeiter von sieben Intensivstationen eines deutschen Universitätsklinikums retrospektiv befragt. Es nahmen 65 Pflegende und 15 Ärzte teil, was einer Rücklaufquote von 30% entspricht. Als Hauptergebnisse wurde festgestellt, dass die Entscheidung zur Therapiedeeskalation laut 96% der Angaben von einem Oberarzt getroffen wurde, die behandelnde Pflegekraft war gemäß 75% der Teilnehmer regelmäßig daran beteiligt. Der Patientenwille wurde gemäß 96% der Antworten im Angehörigengespräch ermittelt. Als häufigste Form der Therapiedeeskalation wurde der Verzicht auf Ausweitung der kurativen Maßnahmen genannt. Gemäß etwa 80-90% der Befragten waren die Symptome der Patienten nach EOL-Entscheidung oft oder immer kontrolliert. Über 90% der Befragten betrachteten die Angehörigen als oft oder immer zufrieden mit Patientenversorgung. Den Wunsch nach Unterstützungsangeboten äußerten 25% der Befragten oft oder immer, am häufigsten nach Teambesprechungen vor Therapiezieländerung. Die Ergebnisse der qualitativen Inhaltsanalyse ergänzten die Resultate aus den quantitativen Items. Signifikante Ergebnisse in den Hypothesentests wiesen auf Diskrepanzen in den Einschätzungen der Berufsgruppen hin, mit tendenziell kritischerer Bewertung bei den Pflegenden.
Eine wesentliche Limitation der vorliegenden Studie liegt in der Rücklaufquote von 30%, die die Repräsentativität der Ergebnisse einschränkt. Außerdem handelt es sich um retrospektive subjektive Einschätzungen, teilweise Fremdbeurteilungen. Eine systematische Reliabilitäts- und Validitätsprüfung des Fragebogens steht aus.
Arrhythmogenic cardiomyopathy has been clinically defined since the 1980s and causes right or biventricular cardiomyopathy associated with ventricular arrhythmia. Although it is a rare cardiac disease, it is responsible for a significant proportion of sudden cardiac deaths, especially in athletes. The majority of patients with arrhythmogenic cardiomyopathy carry one or more genetic variants in desmosomal genes. In the 1990s, several knockout mouse models of genes encoding for desmosomal proteins involved in cell–cell adhesion revealed for the first time embryonic lethality due to cardiac defects. Influenced by these initial discoveries in mice, arrhythmogenic cardiomyopathy received an increasing interest in human cardiovascular genetics, leading to the discovery of mutations initially in desmosomal genes and later on in more than 25 different genes. Of note, even in the clinic, routine genetic diagnostics are important for risk prediction of patients and their relatives with arrhythmogenic cardiomyopathy. Based on improvements in genetic animal engineering, different transgenic, knock-in, or cardiac-specific knockout animal models for desmosomal and nondesmosomal proteins have been generated, leading to important discoveries in this field. Here, we present an overview about the existing animal models of arrhythmogenic cardiomyopathy with a focus on the underlying pathomechanism and its importance for understanding of this disease. Prospectively, novel mechanistic insights gained from the whole animal, organ, tissue, cellular, and molecular levels will lead to the development of efficient personalized therapies for treatment of arrhythmogenic cardiomyopathy.
Die primäre Nebenniereninsuffizienz stellt ein seltenes Krankheitsbild dar, welches mit einem Mangel an Kortisol in Folge eines defekten bzw. fehlenden Nebennierenrindengewebes einhergeht. Im Rahmen eines autoimmun polyglandulären Syndroms Typ 2 (APS 2) können, neben anderen Kombinationsmöglichkeiten, eine primäre Nebenniereninsuffizienz (NNRI) zusammen mit einem Diabetes mellitus Typ 1 vorliegen. Bisher wenig bis gar nicht beleuchtet ist der Einfluss dieser Erkrankungen auf die kardiopulmonale Leistungsfähigkeit aus sportmedizinischer Sicht. Mit Kortisol als ein komplexer Modulator verschiedenster Stoffwechselprozesse und Anpassungen des Körpers an physische wie psychische Belastung ist ein entsprechender Einfluss auf kardiopulmonale Belastungsgrößen zu vermuten.
In der hier vorgestellten Arbeit wurde ein Testprotokoll zur standardisierten und vergleichbaren körperlichen Belastung auf einem Fahrradergometer erstellt und Patienten mit primärer NNRI, APS 2, Diabetes Typ 1 sowie gesunde Kontrollprobanden mittels dieses Protokolls körperlich belastet. Dabei wurden unter kontinuierlicher spirometrischer und elektrokardiographischer Überwachung verschiedene Parameter der Herzfrequenzvariabilität sowie die Slow Component der Sauerstoffaufnahme (SCVO2) beleuchtet und zwischen den einzelnen Gruppen verglichen.
Bei den im Rahmen dieser Arbeit erhobenen HRV-Parametern in Ruhe, bei körperlicher Belastung und in der anschließenden Erholungsphase konnte kein signifikanter Unterschied zwischen den Probandengruppen gesehen werden.
Für die SCVO2 zeigte sich ein signifikanter (p<0,05) Unterschied zwischen Diabetikern und Nicht-Diabetikern. Probanden mit Diabetes wiesen dabei signifikant niedrigere Werte der SCVO2 auf im Vergleich zu Probanden ohne Diabetes. Darüber hinaus zeigte sich eine negative Korrelation zwischen dem HbA1c und der SCVO2.
Ursächlich hierfür könnte ein verändertes Rekrutierungsverhalten von Skelettmuskelfasern sein, wobei zur genaueren Beleuchtung weitere Untersuchungen notwendig sind.
Der primäre Hyperaldosteronismus ist die häufigste Ursache einer sekundären Hypertonie. Ein Aldosteronüberschuss kann in vitro und in vivo DNA-Schäden verursachen. In einzelnen Fallberichten wurde über eine Koinzidenz des primären Hyperaldosteronismus mit Nierenzellkarzinomen wie auch anderen Tumoren berichtet. Die Prävalenz benigner und maligner Neoplasien bei Patienten mit primärem Hyperaldosteronismus wurde bislang jedoch noch nicht untersucht.
In der multizentrischen MEPHISTO-Studie wurde die Prävalenz von benignen und malignen Tumorerkrankungen bei 338 Patienten mit bestätigtem primärem Hyperaldosteronismus untersucht. Hypertensive Probanden der populationsbasierten SHIP-Studie dienten als gematchte Kontrollgruppe.
Bei 120 (35,5%) der 338 Patienten mit primärem Hyperaldosteronismus war eine Tumorerkrankung diagnostiziert worden, 30 der Patienten hatten zwei oder mehr Neoplasien. Das Auftreten eines Malignoms wurde bei 9,5% der Patienten mit primärem Hyperaldosteronismus und bei 6,0% der hypertensiven Kontrollgruppe dokumentiert (p=0,08). Patienten mit primärem Hyperaldosteronismus und einem Malignom in der Vorgeschichte wiesen bei der Diagnose des primären Hyperaldosteronismus höhere Aldosteronspiegel auf (p=0,009). Es konnte zudem ein signifikanter Zusammenhang zwischen der Höhe des Aldosteronspiegels und der Malignomprävalenz beobachtet werden (p=0,03). Insgesamt wurden unter den Patienten mit primären Hyperaldosteronismus 158 Neoplasien nachgewiesen, die in 62% der Fälle benigne und in 25% der Fälle maligne waren (14% Neoplasien unbekannter Dignität). Ein Nierenzellkarzinom wurde bei 5 Patienten (13% aller malignen Tumore) diagnostiziert, in der hypertensiven Kontrollgruppe trat kein Nierenzellkarzinom auf.
Im Vergleich zur hypertensiven Kontrollgruppe wurde bei Patienten mit primärem Hyperaldosteronismus ein Trend hin zu einer erhöhten Prävalenz von Malignomen beobachtet, der beim Nierenzellkarzinom besonders deutlich ausgeprägt und signifikant mit der Höhe des Aldosteronspiegels assoziiert war.
Inherited cardiomyopathies are characterized by clinical and genetic heterogeneity that challenge genetic diagnostics. In this study, we examined the diagnostic benefit of exome data compared to targeted gene panel analyses, and we propose new candidate genes. We performed exome sequencing in a cohort of 61 consecutive patients with a diagnosis of cardiomyopathy or primary arrhythmia, and we analyzed the data following a stepwise approach. Overall, in 64% of patients, a variant of interest (VOI) was detected. The detection rate in the main sub-cohort consisting of patients with dilated cardiomyopathy (DCM) was much higher than previously reported (25/36; 69%). The majority of VOIs were found in disease-specific panels, while a further analysis of an extended panel and exome data led to an additional diagnostic yield of 13% and 5%, respectively. Exome data analysis also detected variants in candidate genes whose functional profile suggested a probable pathogenetic role, the strongest candidate being a truncating variant in STK38. In conclusion, although the diagnostic yield of gene panels is acceptable for routine diagnostics, the genetic heterogeneity of cardiomyopathies and the presence of still-unknown causes favor exome sequencing, which enables the detection of interesting phenotype–genotype correlations, as well as the identification of novel candidate genes.
Das Nebennierenrindenkarzinom (Adrenocortical Cancer, ACC) ist eine seltene Erkrankung mit schlechter Prognose vor allem in fortgeschrittenen Tumorstadien. Diese Studie untersucht ein großes Kollektiv (n=114) an Patienten mit fortgeschrittenem ACC, die mit einer Gemcitabin (Gem)-basierten Therapie behandelt wurden. Hier zeigte sich eine moderate Wirksamkeit mit einem „clinical benefit“ (definiert als ein progressionsfreies Überleben von vier oder mehr Monaten) bei 18% der Patienten und einer guten Verträglichkeit (schwerwiegende unerwünschte Ereignisse bei 11% der Patienten).
Als mögliche prädiktiver Marker für die Gem-Wirksamkeit und prognostische Marker für das Gesamt-Überleben wurden die große Untereinheit M1 der Ribonukleotidreduktase (RRM1) und der Transmembrantransporter human Equilibrative Nucleosie Transporter 1
(hENT1) untersucht. Keiner der Marker hatte einen prädiktiven Vorhersagewert auf das Ansprechen auf die Gem-basierte Therapie oder das progressionsfreie Überleben nach Beginn der Gem-basierten Therapie.
Für die Untersuchung des prognostischen Werts wurden weitere Gewebe untersucht (hENT1 und RRM1 mRNA: 30 ACC, 20 adrenokortikale Adenome
(ACA) and 21 normale Nebennieren (nNN), Gewebe für
hENT1-Proteinuntersuchung: 303 ACC von 262 Pateinten, 51 ACA, 18 nNN, für RRM1-Proteinuntersuchung: 279 ACC von 238 Patienten, 45 ACA, 12nNN).
Es fand sich signifikant weniger hENT1-mRNA und häufiger eine negative hENT1-Protein-Membranfärbung (hENT1m) in ACCs als in ACAs und nNN und tendenziell häufiger negatives hENT1m in Metastasen im Vergleich zu Primärtumoren und Rezidiven. Patienten mit negativem hENT1m hatten ein kürzeres krankheitsspezifisches Überleben und progressionsfreies Überleben unabhängig von der Therapie als Patienten mit positivem hENT1m. Die RRM1-Expression korrelierte nicht mit den Überlebenszeiten unabhängig von der Therapie.
Zusammenfassend zeigt eine Gem-basierte Therapie bei Patienten mit fortgeschrittenem ACC eine moderate Wirksamkeit und einer guten Verträglichkeit. Weder hENT1 noch RRM1 eignen sich als prädiktive Markert für die Wirksamkeit einer Gem-basierten Therapie. hENT1 könnte ein möglicher prognostischer Marker für das ACC sein.
Da bei Patienten mit Nebenniereninsuffizienz (NNI) trotz etablierter Substitutionstherapie eine erhöhte Mortalität nachgewiesen wurde, kommt der Prävention von Nebennierenkisen (NNK) eine starke Bedeutung zu. Mithilfe des in der vorliegenden Arbeit evaluierten Schulungsprogrammes konnte die Grundlage für eine künftig optimierte Krisenpräventionsarbeit für Patienten mit NNI in Deutschland geschaffen werden. Kern der Studie war eine standardisierte 90-120-minütige Gruppenschulung, die in acht Zentren durchgeführt wurde. Mittels Fragebogenerhebung zu drei verschiedenen Zeitpunkten (vorher, direkt nachher, 6-9 Monate nachher) wurde der Wissenstand sowie subjektive Einschätzungen der Patienten zum Umgang mit ihrer Erkrankung evaluiert. Die Patienten schnitten nach Teilnahme an einer standardisierten Schulungsveranstaltung im Wissenstest deutlich besser ab und schätzten den eigenen Informationsstatus sowie das subjektive Sicherheitsgefühl als höher ein. Außerdem stieg die Anzahl der Personen, die sich in einer Notfallsituation die Eigeninjektion von Hydrocortison zutrauen würden, signifikant. Damit zeigt das hier vorgestellte interaktive Training einen eindrucksvollen, positiven Effekt auf den Alltag von NNI-Patienten, die dieses zu jedem untersuchten Zeitpunkt in jeweils > 90 % der Fälle als Lebensqualität-verbessernd einstuften. Obwohl sich die Gesamtpunktzahlen im Wissenstest zwischen dem Zeitpunkt direkt nach einer Schulung und nach 6-9 Monaten nicht signifikant unterschieden, war zumindest in den subjektiven Einschätzungsfragen (beispielsweise bzgl. des Informationsstatus, des Sicherheitsgefühls und des Eigeninjektions-Zutrauens) ein statistisch bedeutsamer Rückgang zu verzeichnen. Dies sollte, auch in Zusammenschau mit der Literatur, als Hinweis dafür aufgefasst werden, dass eine einzelne Schulung pro Patient nicht ausreichend ist. Die zumindest jährliche Wiederholung der geschulten Inhalte wäre deshalb zu empfehlen.
Impact of the Chemokine Receptors CXCR4 and CXCR7 on Clinical Outcome in Adrenocortical Carcinoma

Chemokine receptors have a negative impact on tumor progression in several human cancers and have therefore been of interest for molecular imaging and targeted therapy. However, their clinical and prognostic significance in adrenocortical carcinoma (ACC) is unknown. The aim of this study was to evaluate the chemokine receptor profile in ACC and to analyse its association with clinicopathological characteristics and clinical outcome. A chemokine receptor profile was initially evaluated by quantitative PCR in 4 normal adrenals, 18 ACC samples and human ACC cell line NCI-H295. High expression of CXCR4 and CXCR7 in both healthy and malignant adrenal tissue and ACC cells was confirmed. In the next step, we analyzed the expression and cellular localization of CXCR4 and CXCR7 in ACC by immunohistochemistry in 187 and 84 samples, respectively. These results were correlated with clinicopathological parameters and survival outcome. We detected strong membrane expression of CXCR4 and CXCR7 in 50% of ACC samples. Strong cytoplasmic CXCR4 staining was more frequent among samples derived from metastases compared to primaries (p=0.01) and local recurrences (p=0.04). CXCR4 membrane staining positively correlated with proliferation index Ki67 (r=0.17, p=0.028). CXCR7 membrane staining negatively correlated with Ki67 (r=−0.254, p=0.03) but positively with tumor size (r=0.3, p=0.02). No differences in progression-free or overall survival were observed between patients with strong and weak staining intensities for CXCR4 or CXCR7. Taken together, high expression of CXCR4 and CXCR7 in both local tumors and metastases suggests that some ACC patients might benefit from CXCR4/CXCR7-targeted therapy.
Background: Left ventricular hypertrophy (LVH), defined by the left ventricular mass index (LVMI), is highly prevalent in hemodialysis patients and a strong independent predictor of cardiovascular events. Compared to cardiac magnetic resonance imaging (CMR), echocardiography tends to overestimate the LVMI. Here, we evaluate the diagnostic performance of transthoracic echocardiography (TTE) compared to CMR regarding the assessment of LVMI in hemodialysis patients.
Methods: TTR and CMR data for 95 hemodialysis patients who participated in the MiREnDa trial were analyzed. The LVMI was calculated by two-dimensional (2D) TTE-guided M-mode measurements employing the American Society of Echocardiography (ASE) and Teichholz (Th) formulas, which were compared to the reference method, CMR.
Results: LVH was present in 44% of patients based on LVMI measured by CMR. LVMI measured by echocardiography correlated moderately with CMR, ASE: r = 0.44 (0.34-0.62); Th: r = 0.44 (0.32-0.62). Compared to CMR, both echocardiographic formulas overestimated LVMI (mean increment LVMI (ASE-CMR): 19.5 +/- 19.48 g/m(2),p < 0.001; mean increment LVMI (Th-CMR): 15.9 +/- 15.89 g/m(2),p < 0.001). We found greater LVMI overestimation in patients with LVH using the ASE formula compared to the Th formula. Stratification of patients into CMR LVMI quartiles showed a continuous decrease in increment LVMI with increasing CMR LVMI quartiles for the Th formula (p < 0.001) but not for the ASE formula (p = 0.772). Bland-Altman analysis showed that the Th formula had a constant bias independent of LVMI. Both methods had good discrimination ability for the detection of LVH (ROC-AUC: 0.819 (0.737-0.901) and 0.808 (0.723-0.892) for Th and ASE, respectively).
Conclusions: The ASE and Th formulas overestimate LVMI in hemodialysis patients. However, the overestimation is less with the Th formula, particularly with increasing LVMI. The results suggest that the Th formula should be preferred for measurement of LVMI in chronic hemodialysis patients.
Aims
Inhibition of neprilysin and angiotensin II receptor by sacubitril/valsartan (Val) (LCZ696) reduces mortality in heart failure (HF) patients compared with sole inhibition of renin–angiotensin system. Beneficial effects of increased natriuretic peptide levels upon neprilysin inhibition have been proposed, whereas direct effects of sacubitrilat (Sac) (LBQ657) on myocardial Ca\(^{2+}\) cycling remain elusive.
Methods and results
Confocal microscopy (Fluo‐4 AM) was used to investigate pro‐arrhythmogenic sarcoplasmic reticulum (SR) Ca\(^{2+}\) leak in freshly isolated murine and human ventricular cardiomyocytes (CMs) upon Sac (40 μmol/L)/Val (13 μmol/L) treatment. The concentrations of Sac and Val equalled plasma concentrations of LCZ696 treatment used in PARADIGM‐HF trial. Epifluorescence microscopy measurements (Fura‐2 AM) were performed to investigate effects on systolic Ca\(^{2+}\) release, SR Ca\(^{2+}\) load, and Ca\(^{2+}\)‐transient kinetics in freshly isolated murine ventricular CMs. The impact of Sac on myocardial contractility was evaluated using in toto‐isolated, isometrically twitching ventricular trabeculae from human hearts with end‐stage HF. Under basal conditions, the combination of Sac/Val did not influence diastolic Ca\(^{2+}\)‐spark frequency (CaSpF) nor pro‐arrhythmogenic SR Ca\(^{2}\) leak in isolated murine ventricular CMs (n CMs/hearts = 80/7 vs. 100/7, P = 0.91/0.99). In contrast, Sac/Val treatment reduced CaSpF by 35 ± 9% and SR Ca\(^{2+}\) leak by 45 ± 9% in CMs put under catecholaminergic stress (isoproterenol 30 nmol/L, n = 81/7 vs. 62/7, P < 0.001 each). This could be attributed to Sac, as sole Sac treatment also reduced both parameters by similar degrees (reduction of CaSpF by 57 ± 7% and SR Ca2+ leak by 76 ± 5%; n = 101/4 vs. 108/4, P < 0.01 each), whereas sole Val treatment did not. Systolic Ca2+ release, SR Ca\(^{2+}\) load, and Ca\(^{2+}\)‐transient kinetics including SERCA activity (k\(_{SERCA}\)) were not compromised by Sac in isolated murine CMs (n = 41/6 vs. 39/6). Importantly, the combination of Sac/Val and Sac alone also reduced diastolic CaSpF and SR Ca\(^{2+}\) leak (reduction by 74 ± 7%) in human left ventricular CMs from patients with end‐stage HF (n = 71/8 vs. 78/8, P < 0.05 each). Myocardial contractility of human ventricular trabeculae was not acutely affected by Sac treatment as the developed force remained unchanged over a time course of 30 min (n trabeculae/hearts = 3/3 vs. 4/3).
Conclusion
This study demonstrates that neprilysin inhibitor Sac directly improves Ca\(^{2+}\) homeostasis in human end‐stage HF by reducing pro‐arrhythmogenic SR Ca\(^{2+}\) leak without acutely affecting systolic Ca\(^{2+}\) release and inotropy. These effects might contribute to the mortality benefits observed in the PARADIGM‐HF trial.
Platinbasierte Chemotherapie ist die effektivste Chemotherapie für das fortgeschrittene Nebennierenrindenkarzinom (ACC). Excision repair cross complementing group 1 (ERCC1) spielt eine entscheidende Rolle bei der Reparatur durch Platin entstandener DNA-Schäden. Zwei Studien die die Rolle von ERCC1 per Immunhistochemie als prädiktiver Marker für das Ansprechen auf platinbasierte Chemotherapie beim ACC untersuchten kamen zu sich widersprechenden Ergebnissen. Beide Studien nutzten den ERCC1-Antikörper Klon 8F1 der sich später als nicht spezifisch herausstellte. Das Ziel der Doktorarbeit war die Evaluation von ERCC1 mit einem neuen hoch spezifischen Antikörper in einer großen Kohorte von Patienten mit ACC.
Material und Methoden: 146 Patienten mit verfügbaren FFPE-Schnitten wurden eingeschlossen. Alle Patienten erhielten eine platinbasierte Chemotherapie im Median für 6 Zyklen, nämlich Cisplatin (n=131) oder Carboplatin (n=15), in den meisten Fällen in Kombination mit Etoposid (n=144 , Doxorubicin (n=131) und Mitotane (n=131). Die Immunhistochemische Färbung wurde mit dem neuen Antikörper Klon 4F9 durchgeführt. Der Zusammenhang zwischen ERCC1-Expression und klinischen Parametern, Therapieansprechen, progressionsfreiem Überleben und Gesamtüberleben wurde analysiert.
Ergebnisse: Eine hohe ERCC1-Expression wurde in 66% der Tumorproben beobachtet. Während der Chemotherapie zeigte sich bei 43 Pateinten ein Therapieansprechen (29,5%), bei 49 Patienten eine Stabilisierung der Erkrankung (33,6%), bei 8 Patienten ein gemischtes Ansprechen (5,5%) und bei 46 Pateinten ein Krankheitsprogress (31,5%), ohne Zusammenhang zur ERCC1-Expression. Auch zeigte sich kein signifikanter Zusammenhanf zwischen der ERCC1-Expression und dem progressionsfreien Überleben (Median 6.5 vs. 6 Monate, P=0.33, HR=1.23, 95%CI=0.82-2.0) oder dem Gesamtüberleben.
Zusammenfassung: Es besteht kein Zusammenhang zwischen der ERCC1-Expression und der Platinsensitivität beim Nebennierenrindenkarzinom. Somit werden andere Biomarker zur Therapieentscheidung benötigt.
Morbus Fabry ist eine multisystemische Erkrankung, die zahlreiche Komplikationen mit sich bringt. Eine verminderte Aktivität des Enzyms α-Galactosidase führt zu einer Anreicherung von Globotriaosylceramiden in verschiedenen Organen und Geweben. Betroffene Patienten entwickeln häufig eine linksventrikuläre Hypertrophie und eine renale Fibrose. Wegen Hitzeintoleranz und Hypohidrosis vermeiden Patienten häufig eine Sonnenexposition. Wir stellten die Hypothese auf, dass ein Vitamin D-Mangel an der Ausprägung einer Fabry-kardiomyopathie beteiligt ist.
In dieser Querschnittstudie wurde der Einfluss des Vitamin D-Status auf die typischerweise auftretenden Komplikationen der Erkrankung an 111 Patienten mit gesichertem Morbus Fabry vor Beginn einer Enzymersatztherapie untersucht. Dafür bestimmten wir das 25(OH)D und teilten die Patienten anhand dieser Werte in drei Gruppen ein: Vitamin D-defizient (25(OH)D<15 ng/ml), Vitamin D-insuffizient (25(OH)D 15-30 ng/ml und Vitamin Dsuffizient (25(OH)D >30 ng/ml). Es erfolgten Magnetresonanztomographie- und echokardiographische Untersuchungen zur Bestimmung der linksventrikulären Masse und einer möglichen Kardiomyopathie. In Querschnittsanalysen wurden Assoziationen zwischen auftretenden klinischen Symptomen und dem Vitamin D-Status durch lineare bzw. binäre logistische Regressionsanalysen bestimmt und nach Alter, Geschlecht, BMI und Jahreszeit der Messung adjustiert. Die Patienten waren im Durchschnitt 40,1±12,5 Jahre alt (42% männlich) und hatten einen durchschnittlichen 25(OH)D-Wert von 23,5±11,4 ng/ml. Patienten der defizienten 25(OH)D-Gruppe hatten ein adjustiertes 6-fach erhöhtes Risiko für eine hypertrophe Kardiomyopathie verglichen mit der suffizienten Kontrollgruppe (p=0,04). Die durchschnittliche linksventrikuläre Masse unterschied sich signifikant: 170±75 g in der defizienten, 154±60 g in der insuffizienten und 128±58 g in der suffizienten Gruppe (p=0,01). Mit dem Schweregrad der Vitamin D-Defizienz stieg der Median einer vorhandenen Proteinurie sowie die Prävalenz von Hitzeintoleranz, Ödemen, Cornea
verticillata, Diarrhoen und dem Bedarf einer Schmerzmedikation. Unsere Studie hat gezeigt, dass Patienten mit niedrigen Vitamin D-Werten gehäuft an Komplikationen leiden.75 Dazu gehören die progressive Fabry-Nephropathie, hypertrophe Kardiomyopathie und die Krankheit typischerweise begleitende klinische Symptome. Zum jetzigen Zeitpunkt können wir allerdings eine Vitamin D-Therapie, basierend auf unseren Daten noch nicht grundsätzlich empfehlen obwohl eine Vitamin D-Supplementierung über Vitamin D angereicherte Nahrungsmittel im Allgemeinen empfohlen wird. Unsere Ergebnisse sind bisher nur Beobachtungen und können einen kausalen Zusammenhang nicht beweisen.
Der implantierbare Kardioverter/Defibrillator wird zum Schutz vor einem plötzlichen Herztod aufgrund maligner Herzrhythmusstörungen eingesetzt. Detektionsalgorithmen sollen dabei den Herzrhythmus korrekt klassifizieren und bei ventrikulären Tachykardien eine Therapie mittels Überstimulation oder Schockabgabe auslösen, nicht jedoch bei tachykardem Vorhofflimmern. Die Vermeidung solcher inadäquater Therapieabgaben stellt eine Herausforderung an die Detektionsalgorithmen der Implantate da.
Bei steigender Prävalenz des Vorhofflimmerns auch bei ICD-Trägern stellt sich daher die Frage nach dem Einfluss dieser Rhythmusstörung auf die Versorgung dieser Patientengruppe.
In der vorliegenden Arbeit sind retrospektiv die Daten von ICD-Trägern mit Vorhofflimmern und ICD-Trägern ohne Vorhofflimmern bezüglich der gewählten Implantate, Häufigkeit adäquater und inadäquater Therapieabgaben und der daraus resultierenden Maßnahmen untersucht worden.
In der Auswertung zeigt sich, dass Patienten mit Vorhofflimmern tendenziell mehr inadäquate Therapien erlitten und signifikant häufiger hospitalisiert werden mussten. Anhand der Daten zeigt sich, dass Vorhofflimmern maßgeblich die Versorgung dieser Patientengruppen beeinflusst.
Der implantierbare Defibrillator hat sich als wichtigstes Instrument bei der Prävention des plötzlichen Herztodes auf dem Boden maligner Arrhythmien erwiesen3. Dennoch gibt es Patienten, die niemals eine lebensrettende Therapie durch ihr ICD-Aggregat erfahren.
Ziel der vorliegenden retrospektiven, monozentrischen Studie war es, den Nutzen eines ICD-Aggregatwechsels trotz ersten Aggregatlebens ohne adäquate Therapieabgaben zu eruieren. Mit dieser Zielsetzung wurde untersucht, welche Patienten von einem Aggregatwechsel profitieren und welche nicht. Eingeschlossen wurden 71 Patienten, die mindestens einen ICD-Aggregatwechsel durchlaufen und keine adäquate Therapie durch ihr erstes Aggregat erfahren hatten.
Die statistische Auswertung der erhobenen Daten ergab mehrere signifikante Prädiktoren für die Notwendigkeit einer ICD-Therapie nach dem ersten Aggregatwechsel trotz ersten Aggregatlebens ohne adäquate Therapieabgaben:
Hauptprädiktoren waren eine hohe Anzahl an kardiovaskulären Risikofaktoren, nicht therapierte, aber doch dokumentierte ventrikuläre Rhythmusstörungen, eine geringe linksventrikuläre Ejektionsfraktion (LVEF) zum Zeitpunkt des ersten Aggregatwechsels sowie eine kurze Lebensdauer des ersten ICD-Aggregats.
Weitere signifikante Prädiktoren waren Adipositas Grad II, Niereninsuffizienz Grad III, nicht-insulinpflichtiger Diabetes mellitus, Apoplex, Hyperurikämie sowie Phenprocoumon-Therapie.
Aktuell fehlen Daten und klare Leitlinien, die sich nicht nur wie bisher auf die Indikationsstellung bei Primärimplantation beziehen, sondern die Entscheidungshilfen geben, ob ein Aggregatwechsel durchgeführt werden sollte oder nicht. Die Ergebnisse dieser Arbeit unterstützen diese Forderung nach neuen Leitlinien.
Die Sepsis ist ein häufiges, komplexes Krankheitsbild und oft mit einer hohen Letalität verbunden. Um das Outcome der betroffenen Patienten zu verbessern, ist eine schnelle adäquate Therapie notwendig. Durch den schnellen Erregernachweis ist eine gezielte Antibiotikatherapie möglich. In der vorliegenden Arbeit wurden 57 Patienten der IMPACT-Sepsis Studie, die bereits antibiotisch vorbehandelt waren, hinsichtlich der Erregerdiagnostik mit dem aktuellen Goldstandard, der Blutkulturdiagnostik, und der VYOO®-Multiplex-PCR untersucht.
Das Patientenkollektiv war epidemiologisch vergleichbar mit dem anderer Studien, nur lag der Anteil an immunsupprimierten Patienten höher, was a. e. auf das Patientenkollektiv einer Universitätsklinik zurückzuführen ist.
Insgesamt konnten bei den Patienten 21 Erreger diagnostiziert werden. 10 Erreger wurden nur in der VYOO®-Multiplex-PCR, nur 3 in der Blutkulturdiagnostik und 4 Erreger in beiden Methoden nachgewiesen. Dies entspricht einer Nachweisquote pro Patient von 21,4% für die VYOO®-Multiplex-PCR und 12,5% für die Blutkulturdiagnostik. Es zeigte sich somit eine tendenziell bessere Detektionsrate bei der VYOO®-Multiplex-PCR. Verglichen mit Blutkulturdiagnostik gab es jedoch keine statistisch signifikante Verbesserung der Erregerdiagnostik mit der VYOO®-Multiplex-PCR. Dies ist a.e. auf eine kleine Studiengröße zurückzuführen.
Wird die aktuelle Studienlage betrachtet, so bleibt die Blutkulturdiagnostik bei septischen Patienten weiterhin der Goldstandard, was sich auch in den aktuellen Leitlinien von 2016 widerspiegelt. Ob molekulardiagnostische Verfahren, wie die PCR, die Blutkulturdiagnostik ablösen oder routinemäßig ergänzen werden, müssen weitere Studien zeigen. Auch der gesundheits-ökonomische Nutzen durch Verkürzung der intensivmedizinischen Behandlung und Reduktion des Antibiotikaverbrauchs bedarf prospektiver Untersuchungen eines großen Patientenkollektivs.
Für die SONAR werden Patienten mit Diabetes mellitus Typ 2, diabetischer Nephropathie (eGFR 25-75ml/min/1,73m2) und Makroalbuminurie (UACR ≥300 und <5000mg/g) gesucht. Die Studie im Enrichment Responder Design soll die Auswirkungen des selektiven Endothelin-Antagonisten Atrasentan aufzeigen. Die Einschlusskriterien selektieren aus dem großen Patientenkollektiv der Typ-2-Diabetiker diejenigen mit fortgeschrittenem Progressionsstadium der diabetischen Nephropathie. Für die Optimierung und effizientere Suche werden Konzepte erstellt. Die Anzahl gefundener Patienten bleibt unter den Erwartungen. Die Rekrutierungszahlen der anderen deutschen Prüfzentren werden erhoben und ausgewertet, der Verbleib der Makroalbuminuriker und die Notwendigkeit neuer Therapieoptionen zur Progressionsverzögerung der diabetischen Nephropathie diskutiert.
Abstract:
Hintergrund: Die Nierentransplantation bietet einer wachsenden Zahl älterer Patienten mit terminaler Niereninsuffizienz ein verbessertes Überleben und eine höhere Lebensqualität. Ältere Empfänger von Nierentransplantaten können jedoch aufgrund einer höheren Komorbiditätsbelastung, eines höheren kumulativen Risikos von Komplikationen im Zusammenhang mit der Immunsuppression und einer zunehmenden Gebrechlichkeit vor besondere Herausforderungen gestellt werden. Dennoch ist wenig über den Einfluss des Alterns auf die Situation dieser wachsenden Klientel bekannt.
Diese Dissertation zielt darauf ab, die Überzeugungen und Werte, Einstellungen und Perspektiven älterer Nierentransplantatempfänger im oder über dem 60. Lebensjahr hinsichtlich gesundheitsbezogener Lebensqualität, Krankheitsbewältigung und Selbstmanagement beim Leben und Altern mit einem Nierentransplantat zu beschreiben, um die patientenzentrierte Nachsorge zu verbessern.
Methoden der systematischen qualitativen Übersicht: Bis April 2015 wurden elektronische Datenbanken durchsucht. Qualitative Studien waren verwendbar, wenn sie von den Einstellungen älterer Nierentransplantempfänger berichteten (≥ 60 Jahre). Zur Analyse der Ergebnisse wurde eine thematische Synthese verwendet.
Methodik der Interviewstudie: Es wurden persönliche, semi-strukturierte Interviews mit 30 Empfängern von Nierentransplantaten im Alter von 65 bis 80 Jahren aus fünf nephrologischen Einrichtungen in Australien durchgeführt. Die Transkripte wurden thematisch analysiert.
Ergebnisse der systematischen qualitativen Übersicht: 21 Studien mit mehr als 116 Empfängern wurden einbezogen. Es wurden sieben Themen identifiziert. Die Wiedererlangung von Stärke und Vitalität bedeutete, die physischen und psychosozialen Verbesserungen im Alltagsleben zu schätzen. Verlängerung des Lebens bezog sich auf die Bereitschaft, jedes Tranplantatorgan (notfalls auch eines, das nicht höchsten Qualitätskriterien entsprach) zu akzeptieren, um das Überleben zu verlängern. Dankesschuld bedeutete eine bewusste Wertschätzung der Transplantatempfänger gegenüber ihrem Spender, wobei sie wussten, dass sie dessen Opfer nicht zurückzahlen konnten. Moralische Verantwortung für den Erhalt der Gesundheit motivierte zur Einhaltung von Medikamenten- und Verhaltensempfehlungen aus einer ethischen Verpflichtung heraus, das Überleben des gespendeten Organs zu schützen. Anhaltende und zunehmende Vergesslichkeit behinderte die Selbstorganisation, Enttäuschung über Nebenwirkungen und Komplikationen spiegelte Frustration und Wut über die unbeabsichtigten Folgen der Medikation wider. Die Endgültigkeit der Behandlungsmöglichkeit bezog sich auf das Bewusstsein, dass die aktuelle Transplantation die letzte mögliche gewesen sein könnte.
Ergebnisse der Interviewstudie: Sechs Themen wurden identifiziert: Wiedererlangen jugendlicher Vitalität (mit Unterthemen: sich erholende Widerstandsfähigkeit, umfassende Lebensfreude, Drang zur Selbstverwirklichung); Hartnäckiges Überstehen der langwierigen Genesung (dem Altern nachgeben, funktionelle Einschränkungen akzeptieren, Grenzen überschreiten, dauerhafte Behandlungsverantwortung); Überlagerung durch andere Erkrankungen (Bekämpfung verheerender Komorbiditäten, schmerzhafte Einschränkungen, aufkommende Desillusionierung, Ängste wegen sich häufender Nebenwirkungen, aufzehrende Behandlungslast); Priorisieren des Transplantatüberlebens (privilegiert mit einem Wunder, Inkaufnehmen von Risiken für die Langlebigkeit, Erfüllen einer moralischen Verpflichtung, Bewahrung der letzten Gelegenheit); Konfrontation mit Gesundheitsverschlechterung (Verletzlichkeit und Hilflosigkeit, Verengung des Fokus auf unmittelbare Sorgen, Überlebensungewissheit); und Daseinswert (Lebenssinn durch Autonomie, Ablehnung der Last von vergeblicher Behandlung, Überleben unter allen Umständen).
Schlussfolgerung:
Die Transplantation im höheren Alter schenkt älteren Nierentransplantatempfängern Lebenskraft und Vitalität. Dankbar für den Überlebensgewinn, nutzen sie ihre Kraft, um ihre Gesundheit und die des Transplantats so lange wie möglich zu erhalten. Jedoch bedrohen eine langwierige Genesung, anhaltende und zunehmende Vergesslichkeit und Komplikationen durch chronische Komorbiditäten und Behandlungsnebenwirkungen die wiedererlangte Zufriedenheit und Lebensfreude. Enttäuschung entsteht über eine nicht enden wollende Behandlungslast. Einige renale Transplantatempfänger sehen ihr jetziges Transplantat als letzte Chance an und verknüpfen dies mit dem Wert ihres Daseins.
Unterstützung während eines langen Genesungsprozesses, das Aufrechterhalten von funktionalen Fähigkeiten, die Therapie behandelbarer Nebenwirkungen und die Wahrnehmung von Perspektiven, Zielen und Werteinstellungen beim Empfänger können die patientenorientierte Versorgung verbessern und älteren Empfängern ermöglichen, ihr Transplantat und ihre Lebensqualität zu erhalten.
Das Ziel dieser klinisch retrospektiven Studie ist es die Prävalenz der aufgrund eines zu hohen Mortalitätsrisikos für eine Operation nicht geeigneten Patienten mit hochgradiger MI für ein kathetergestütztes Verfahren namens „Mitraclip“, das in Europa erst seit 2008 als optionale Therapieform besteht, zu bestimmen. Dabei interessiert man sich für die Häufigkeit der hochgradigen MI in einem Krankenhaus der Maximalversorgung. Anhand dieser Daten kann sowohl die Prognose der Patienten mit diesem Krankheitsbild bestimmt, sowie die Prävalenz der geeignetsten Therapieform (chirurgisch, interventionell, medikamentös) für jeden einzelnen Patienten erhoben werden.
Methoden:
Mit einer von dem Herzinsuffizienzzentrum der Universitätsklinik Würzburg konzipierten Software namens DataWarehouse konnte mit einer Präzisionsrate von 0,99 aus einer Datenbank von 800.000 Patienten ein Kollektiv mit mittel bis hochgradiger Mitralklappeninsuffzienz und einem Mindestalter von 18 Jahren, in einem Zeitraum zwischen 2008 und 2012, identifiziert werden. Ebenfalls mit eingeschlossen wurden Patienten mit einem Zustand nach Mitralklappenrekonstruktion oder Klappenersatz. Hierbei bediente sich die Software der Informationen aus Arztbriefen, echokardiographischen Befunden, Koronarangiographien, Magnetresonanztomographien und Anamnesen. Zur Minimierung der möglichen Computerfehler, wurden die identifizierten Probanden nochmals manuell überprüft und nach den echokardiographischen Kriterien für eine mittel-hochgradige MI, anhand der aus dem klinischen Routineuntersuchung gespeicherten Ultraschallbildern im EchoPac, nachgemessen. Des Weiteren wurden im Rahmen der klinischen Routineuntersuchung echokardiographische Verlaufskontrollen inclusive Blutbild und 6-Minuten-Gehtest, durchgeführt und zur Einordnung der Insuffizienz und zur Evaluation der Morphologie der Mitralsegel für die Option des kathetergestützten Verfahrens herangezogen. Schlussendlich wurden alle für eine Operation geeigneten Patienten anhand der Verlaufsprotokolle in den Arztbriefen bestimmt und die konservative Gruppe nach der Segelmorphologie für ein minimalinvasives Verfahren, oder einem palliativen Umgang eingeteilt. Da die Einteilung nicht extern geprüft worden ist, sollte der STS Score, der zur Bestimmung des perioperativen Mortalitätsrisikos anhand der Risikofaktoren berechnet wird, die Tendenz der Einteilung evaluieren.
Ergebnisse:
Aus 43.690 Patienten, die in einer Periode von 4 Jahren in der Universitätsklinik Würzburg stationär oder ambulant behandelt worden sind, wurden 2305 (5,3%) Patienten mit einer mittel, mittel-hochgradigen oder hochgradigen MI identifiziert. Nach manueller Überprüfung erwiesen sich die Mitralklappen von 387 Patienten als mittel-hochgradig oder hochgradig und laut der echokardiographischen Auswertungen hatten letzten Endes 331 Probanden eine hochgradige MI. Von den 331 Patienten wurden 125 (38%) operativ versorgt und 206 (62%) konservativ medikamentös behandelt. 70% davon wiesen eine funktionelle MI auf. Von den 206 medikamentös behandelten Patienten hat sich der Schweregrad der MI bei 19 (16%) mit Hilfe der Medikamente gebessert. Nichtsdestotrotz verstarben weitere 90 (44%) bereits innerhalb von 23 Monaten. Dabei wären 81 (39%) Patienten der konservativen Gruppe für ein Mitraclip Verfahren geeignet gewesen und weitere 34 (17%) hätte man einem operativen Verfahren unterziehen können. Die restlichen 72 (35%) Patienten wurden wegen des zu hohen Mortalitätsrisikos und der ungeeigneten Segelmorphologie palliativ eingestuft. Das Ziel dieser klinisch retrospektiven Studie ist es die Prävalenz der aufgrund eines zu hohen Mortalitätsrisikos für eine Operation nicht geeigneten Patienten mit hochgradiger MI für ein kathetergestütztes Verfahren namens „Mitraclip“, das in Europa erst seit 2008 als optionale Therapieform besteht, zu bestimmen. Dabei interessiert man sich für die Häufigkeit der hochgradigen MI in einem Krankenhaus der Maximalversorgung. Anhand dieser Daten kann sowohl die Prognose der Patienten mit diesem Krankheitsbild bestimmt, sowie die Prävalenz der geeignetsten Therapieform (chirurgisch, interventionell, medikamentös) für jeden einzelnen Patienten erhoben werden.

Das Multiple Myelom stellt in Deutschland die zweithäufigste hämatologische Neoplasie dar. Im Rahmen der Induktionstherapie mit anschließender autologer Stammzelltransplantation werden gehäuft kardiotoxische Substanzen eingesetzt; eine der schwerwiegendsten kardialen Nebenwirkung hierdurch ist die Chemotherapie-induzierte Herzinsuffizienz. In bis zu 10 % der Fälle ist das Multiple Myelom mit einer kardialen Amyloidose (AL-Amyloidose) assoziiert. Deren klinischen Symptomatik kann gehäuft ebenfalls mit Zeichen der Herzinsuffizienz einhergehen. Die Echokardiographie stellt für die Diagnostik der Herzinsuffizienz den Goldstandard dar.
Ziel dieser retrospektiven Kohortenstudie (AmcorRetro-Studie) war es durch den Vergleich der echokardiographischen Parameter vor/nach Induktionstherapie mögliche kardiotoxische Effekte und prognostischer Relevanz während der Therapie zu bestimmen. Des Weiteren erfolgte die Evaluation der Prävalenz der kardialen Amyloidose. In einer weiteren Analyse erfolgte pulmonaler Parameter mittels Lungenfunktionsdiagnostik während des Therapieverlaufes. Initial waren 325 Patienten, die im Rahmen der Therapie des Multiplen Myeloms in den Jahren von 2004 bis 2011 mindestens einmal an der Universitätsklinik Würzburg autolog stammzelltransplantiert wurden, eingeschlossen. In der hier vorliegenden Arbeit mit dem Schwerpunkt der echokardiographischen Endpunkte befinden sich 100 Patienten in der Kohorte, bei denen mindestens zwei serielle Echokardiographien (vor und nach Induktion) vorlagen.
In der Analyse der echokardiographischen Parameter (Follow-up im Median 13 Monaten) konnte eine signifikante Reduktion der linken Vorhofparameter nachgewiesen werden (LADs: -1,5 mm, p = 0,009; Septumdicke: - 1 mm, p = 0,001). Es gab keine signifikanten Einflüsse auf die linksventrikuläre Pumpfunktion (vor/nach der Therapie ≥ 55%, p = 0,24), allerdings entwickelten drei Patienten eine Abnahme der systolischen Funktion < 50 % (p = 0,08). Im Gegensatz dazu zeigte sich eine signifikante Zunahme der diastolischen Dysfunktion um 16,5 % (p = 0,002). In der univariaten Cox-Regressionsanalyse war eine höhere systolische Pumpfunktion mit einem signifikanten Überleben assoziiert (HR= 0,89; p = 0,05).
In unser Kohorte hatten sieben Patienten (N = 7/100) vor Induktion den histologischen Nachweis einer Amyloidose (Prävalenz 7 %), in zwei Fällen mit kardialer Beteiligung (N= 2/100).
Im Vergleich der Lungenfunktionsdiagnostik (vor/nach Induktion) zeigte sich eine signifikante Zunahme des Residualvolumens (p = 0,04). In der univariaten Cox- Regressionsanalyse war eine hohe Vitalkapazität sowie hohe Einsekundenkapazität mit einem signifikanten Überlebensvorteil assoziiert. Dieser Trend zeigte sich ebenfalls auch nach trivariater Adjustierung für Alter und Geschlecht (VC in %: HR= 1,0; p= 0,03 und FEV1 in %: HR= 1,0; p = 0,07).
Die Studie konnte mit der hier nachgewiesenen Prävalenz von 7 % bestätigen, dass die kardiale Amyloidose eine seltene Folgeerkrankung des Multiplen Myeloms ist. Die Stammzell-transplantation hat sowohl positive (Reduktion LADs, Septumdicke) als auch negative Auswirkungen (Anstieg diastolische Dysfunktion, im Trend Zunahme der Patienten mit eingeschränkter systolischer Funktion) auf die kardiale Funktion. Die systolische Funktion war als einziges prognoserelevant. Unter der Chemotherapie konnte eine Zunahme des Residualvolumens und somit eine direkte Lungenschädigung beobachtet werden.
Die Mortalitätsrate von Dialysepatienten ist gegenüber der Allgemeinbevölkerung deutlich erhöht; im Besonderen bei Patienten, die zugleich an einem Diabetes mellitus Typ 2 leiden. Es gibt zahlreiche Studien, die versuchen, Prädiktoren für das Kurz- und Langzeitüberleben von Dialysepatienten zu ermitteln. Studien, die dasselbe für hämodialysepflichtige Patienten mit Typ 2 Diabetes mellitus versuchen, sind jedoch noch selten. Ziel der vorliegenden Arbeit war es, die Überlebenden der Deutschen Diabetes Dialyse Studie (4D Studie) zu charakterisieren und Überlebenszeitanalysen durchzuführen, um Hypothesen für zukünftige Studien zu generieren und mögliche Risikofaktoren für eine erhöhte Mortalität zu ermitteln.
In der vorliegenden Arbeit wurden mit Hilfe der Daten der 4D-Studie Überlebenszeitanalysen für 1255 hämodialysepflichtige Patienten mit Diabetes mellitus Typ 2 aus 178 Dialysezentren in Deutschland durchgeführt (mittlere Beobachtungsdauer: 11,5 Jahre). Der primäre Endpunkt war die Gesamtmortalität. Mit dem Cox Proportional Hazards Modell wurden Hazard Ratios für zehn ausgewählte Parameter (Alter, Geschlecht, Pflege, Body Mass Index, Albumin, Diabetesdauer, HbA1c, und kardiovaskuläre, zerebrovaskuläre, periphere vaskuläre Vorerkrankungen) sowohl für das gesamte Kollektiv als auch für nach Geschlecht, Alter und Diabetesdauer aufgeteilte Subgruppen berechnet. Anschließend wurden für alle drei Subgruppen Interaktionsanalysen durchgeführt.
Die Überlebenden des gesamten Studienzeitraumes von 11,5 Jahren sind im Durchschnitt jünger und häufiger männlich, haben weniger Vorerkrankungen und sind seltener pflegebedürftig, sind kürzer an Diabetes mellitus erkrankt, das HbA1c ist besser eingestellt und BMI und Serumalbumin liegen höher als bei den Verstorbenen. Pflegebedürftigkeit (HR = 1,199, p = 0,018), ein niedriges Serumalbumin (HR = 0,723, p = 0,002) sowie kardiovaskuläre (HR = 1,423, p < 0,001) und periphere vaskuläre Erkrankungen (HR = 1,549, p < 0,001) erhöhen das Mortalitätsrisiko signifikant. Weibliche Patienten verlieren ihren, in der Gesamtbevölkerung üblichen, Überlebensvorteil (HR = 1,019, p < 0,79).
