Expression of surfactant protein B is dependent on cell density in H441 lung epithelial cells

Background
Expression of surfactant protein (SP)-B, which assures the structural stability of the pulmonary surfactant film, is influenced by various stimuli, including glucocorticoids; however, the role that cell-cell contact plays in SP-B transcription remains unknown. The aim of the current study was to investigate the impact of cell-cell contact on SP-B mRNA and mature SP-B expression in the lung epithelial cell line H441.
Methods
Different quantities of H441 cells per growth area were either left untreated or incubated with dexamethasone. The expression of SP-B, SP-B transcription factors, and tight junction proteins were determined by qPCR and immunoblotting. The influence of cell density on SP-B mRNA stability was investigated using the transcription inhibitor actinomycin D.
Results
SP-B mRNA and mature SP-B expression levels were significantly elevated in untreated and dexamethasone-treated H441 cells with increasing cell density. High cell density as a sole stimulus was found to barely have an impact on SP-B transcription factor and tight junction mRNA levels, while its stimulatory ability on SP-B mRNA expression could be mimicked using SP-B-negative cells. SP-B mRNA stability was significantly increased in high-density cells, but not by dexamethasone alone.
Conclusion
SP-B expression in H441 cells is dependent on cell-cell contact, which increases mRNA stability and thereby potentiates the glucocorticoid-mediated induction of transcription. Loss of cell integrity might contribute to reduced SP-B secretion in damaged lung cells via downregulation of SP-B transcription. Cell density-mediated effects should thus receive greater attention in future cell culture-based research.
Peroral endoscopic myotomy (POEM) is a new endoscopic treatment for achalasia with very good short-term results in adults. Data about POEM in pediatric patients are missing. We present the case of a 10-year-old male patient with type I (classic) achalasia, successfully treated with POEM. The procedure was accomplished in a similar fashion to the technique used in adults. Short-term results were fine, with a complete control of dysphagia and absence of reflux. We suggest that POEM is a suitable option in pediatric patients—similar to adults—but long-term results must be awaited.
Wilms' tumor primary cells display potent immunoregulatory properties on NK cells and macrophages

The immune response plays a crucial defensive role in cancer growth and metastasis and is a promising target in different tumors. The role of the immune system in Wilm’s Tumor (WT), a common pediatric renal malignancy, is still to be explored. The characterization of the immune environment in WT could allow the identification of new therapeutic strategies for targeting possible inhibitory mechanisms and/or lowering toxicity of the current treatments. In this study, we stabilized four WT primary cultures expressing either a blastematous (CD56\(^+\)/CD133\(^−\)) or an epithelial (CD56\(^−\)/CD133\(^+\)) phenotype and investigated their interactions with innate immune cells, namely NK cells and monocytes. We show that cytokine-activated NK cells efficiently kill WT cells. However, after co-culture with WT primary cells, NK cells displayed an impaired cytotoxic activity, decreased production of IFNγ and expression of CD107a, DNAM-1 and NKp30. Analysis of the effects of the interaction between WT cells and monocytes revealed their polarization towards alternatively activated macrophages (M2) that, in turn, further impaired NK cell functions. In conclusion, we show that both WT blastematous and epithelial components may contribute directly and indirectly to a tumor immunosuppressive microenvironment that is likely to play a role in tumor progression.
Objective
Antinuclear antibody (ANA)–positive juvenile idiopathic arthritis (JIA) is characterized by synovial B cell hyperactivity, but the precise role of CD4+ T cells in promoting local B cell activation is unknown. This study was undertaken to determine the phenotype and function of synovial CD4+ T cells that promote aberrant B cell activation in JIA.
Methods
Flow cytometry was performed to compare the phenotype and cytokine patterns of PD-1\(^h\)\(^i\)\(^g\)\(^h\)CD4+ T cells in the synovial fluid (SF) of patients with JIA and T follicular helper cells in the tonsils of control individuals. TCRVB next-generation sequencing was used to analyze T cell subsets for signs of clonal expansion. The functional impact of these T cell subsets on B cells was examined in cocultures in vitro.
Results
Multidimensional flow cytometry revealed the expansion of interleukin-21 (IL-21) and interferon-γ (IFNγ)–coexpressing PD-1\(^h\)\(^i\)\(^g\)\(^h\)CXCR5–HLA–DR+CD4+ T cells that accumulate in the joints of ANA-positive JIA patients. These T cells exhibited signs of clonal expansion with restricted T cell receptor clonotypes. The phenotype resembled peripheral T helper (Tph) cells with an extrafollicular chemokine receptor pattern and high T-bet and B lymphocyte–induced maturation protein 1 expression, but low B cell lymphoma 6 expression. SF Tph cells, by provision of IL-21 and IFNy, skewed B cell differentiation toward a CD21\(^l\)\(^o\)\(^w\)\(^/\)\(^-\)CD11c+ phenotype in vitro. Additionally, SF Tph cell frequencies correlated with the appearance of SF CD21\(^l\)\(^o\)\(^w\)\(^/\)\(^-\)CD11c+CD27–IgM– double-negative (DN) B cells in situ.
Background
Multiple myeloma (MM) is the third most common hematologic malignancy with increasing importance due to improving treatment strategies and long-term outcomes in an aging population. This study aims to analyse influencing factors on health-related quality of life (HRQoL), such as treatment strategies, participation in a clinical trial and patient characteristics like anxiety, depression, gender, and age. A better understanding of the individual factors in context with HRQoL could provide a helpful instrument for clinical decisions.
Methods
In this prospective observational study, the HRQoL of MM patients with different therapies (first-line and relapse) was quantified by standardized questionnaires (EORTC QLQ-C30 and -MY20) in the context of sociodemographic data, individual anxiety and depressiveness (PHQ-4), and a selected number of clinical parameters and symptoms at defined time-points before, during, and after therapy.
Results
In total, 70 patients were included in the study. The median age of the study cohort was 62 years. 44% were female and 56% were male patients. More than half of the patients were fully active with an ECOG 0. Global health status was significantly higher in patients with first-line treatment and even increased after start of therapy, while the pain level decreased. In contrast, patients with relapsed MM reported a decreasing global health status and increasing pain. Additionally, there was a higher global health status in less anxious/depressive patients. HRQoL decreased significantly after start of chemotherapy in the parameters body image, side effects of treatment, and cognitive functioning. Tandem stem-cell transplantation was not found to be a risk factor for higher impairment of HRQoL. Participation in a clinical study led to an improvement of most aspects of HRQoL. Among others, increased anxiety and depression, female gender, older age, impaired performance status, and recurrent disease can be early indicators for a reduced HRQoL.
Conclusion
This study showed the importance of regular longitudinal assessments of patient reported outcomes (PROs) in routine clinical care. For the first time, to our knowledge, we were able to demonstrate a potential impact between participation in clinical trials and HRQoL. However, due to frequently restrictive inclusion criteria for clinical trials, these MM patients might not be directly comparable with patients treated within standard therapy concepts. Further studies are needed to clarify the relevance of this preliminary data in order to develop an individualized, patient-centred, therapy concept.
Mit dieser Arbeit sollte untersucht werden, ob es eine Beeinträchtigung der körperlichen Leistungsfähigkeit bei Patienten bis zum 20. Lebensjahr mit inaktiver juveniler idiopathischer Arthritis bzw. einer Arthritis in Remission im Vergleich zu gesunden Gleichaltrigen gibt und wenn ja, ob ein Zusammenhang zu dem Eiweißkörper HLA B27 besteht.
This study is aimed at detecting the rate of untimely immunization in a large cohort of extremely low gestational age neonates (ELGANs) of the German Neonatal Network (GNN) and at addressing risk factors for delayed vaccination and associated long-term consequences. We performed an observational study of the GNN between 1st January 2010 and 31st December 2019. The immunization status for the hexavalent and pneumococcal immunization was evaluated in n = 8401 preterm infants <29 weeks of gestation. Univariate analysis and logistic/linear regression models were used to identify risk factors for vaccination delay and outcomes at a 5-year follow-up. In our cohort n = 824 (9.8%) ELGANs did not receive a timely first immunization with the hexavalent and pneumococcal vaccine. Risk factors for delayed vaccination were SGA status (18.1% vs. 13.5%; OR 1.3; 95% CI: 1.1–1.7), impaired growth and surrogates for complicated clinical courses (i.e., need for inotropes, necrotizing enterocolitis). At 5 years of age, timely immunized children had a lower risk of bronchitis (episodes within last year: 27.3% vs. 37.7%; OR 0.60, 95% CI: 0.42–0.86) but spirometry measures were unaffected. In conclusion, a significant proportion of ELGANs are untimely immunized, specifically those with increased vulnerability, even though they might particularly benefit from the immune-promoting effects of a timely vaccination.
Immunization of preterm infants: current evidence and future strategies to individualized approaches

Preterm infants are at particularly high risk for infectious diseases. As this vulnerability extends beyond the neonatal period into childhood and adolescence, preterm infants benefit greatly from infection-preventive measures such as immunizations. However, there is an ongoing discussion about vaccine safety and efficacy due to preterm infants’ distinct immunological features. A significant proportion of infants remains un- or under-immunized when discharged from primary hospital stay. Educating health care professionals and parents, promoting maternal immunization and evaluating the potential of new vaccination tools are important means to reduce the overall burden from infectious diseases in preterm infants. In this narrative review, we summarize the current knowledge about vaccinations in premature infants. We discuss the specificities of early life immunity and memory function, including the role of polyreactive B cells, restricted B cell receptor diversity and heterologous immunity mediated by a cross-reactive T cell repertoire. Recently, mechanistic studies indicated that tissue-resident memory (Trm) cell populations including T cells, B cells and macrophages are already established in the fetus. Their role in human early life immunity, however, is not yet understood. Tissue-resident memory T cells, for example, are diminished in airway tissues in neonates as compared to older children or adults. Hence, the ability to make specific recall responses after secondary infectious stimulus is hampered, a phenomenon that is transcriptionally regulated by enhanced expression of T-bet. Furthermore, the microbiome establishment is a dominant factor to shape resident immunity at mucosal surfaces, but it is often disturbed in the context of preterm birth. The proposed function of Trm T cells to remember benign interactions with the microbiome might therefore be reduced which would contribute to an increased risk for sustained inflammation. An improved understanding of Trm interactions may determine novel targets of vaccination, e.g., modulation of T-bet responses and facilitate more individualized approaches to protect preterm babies in the future.
Prognostischer Einfluss der systemischen Immunsuppression bei Patienten mit hochgradigem Gliom

Beim Glioblastoma multiforme als Neoplasie des zentralen Nervensystems mit sehr schlechtem Outcome sind sowohl auf zellulärer als auch auf humoraler Ebene verschiedene Tumor-Escape-Mechanismen beschrieben. Zur umfassenden Charakterisierung des systemischen Immunstatus wurden bei 61 Patienten mit hochgradigem Gliom entsprechend WHO °III und °IV 30 hämatologische und 36 Plasmamarker prä- und postoperativ sowie im Vergleich mit einer Kontrollgruppe untersucht.
Periphere myeloide und plasmazytoide dendritische Zellen zirkulieren im peripheren Blut in reduzierten Frequenzen, postoperativ ist dies nur noch bei den plasmazytoiden dendritschen Zellen festzustellen. Die durchflusszytometrische Phänotypisierung der dendritschen Zellen ergibt sowohl nativ als auch nach in vitro-Stimulation einen funktionell intakten Phänotyp. Eine Leukozytose und Neutrophilie mit erhöhter Neutrophilen-Lymphozyten-Ratio ist überwiegend durch die perioperative Dexamethason-Applikation zu erklären. Im Gegensatz hierzu korreliert die Lymphopenie insbesondere der T-Zell-Reihe mit dem WHO- Grad. HLA-DR-negative Monozyten im Sinne myeloider Suppressorzellen sind in signifikant erhöhten Frequenzen im peripheren Blut zu beobachten und persistieren nach der chirurgischen Tumorentfernung. Patienten mit Frequenzen oberhalb des Medians haben ein 3,6-fach erhöhtes Risiko für Tumorprogress und ein 2,2-fach erhöhtes Risiko zu versterben.
Bezüglich der Plasmamarker sind beim Gliompatienten signifikant erhöhte Konzentrationen von IL-2, IL-4, IL-5, IL-10 und MACC1 zu messen. Dies verdeutlicht die potentielle Bedeutung von MACC1 als Biomarker in diesem Patientenkollektiv. EGF konnte in der vorliegenden Arbeit mit dem Gesamtüberleben korreliert werden: Eine Verdopplung der Serumkonzentration ist mit einem 1,3-fach erhöhten Sterberisiko assoziiert.
Zusammenfassend unterstreicht diese Arbeit die systemische Kompromittierung des Immunsystems und fügt noch nicht beschrieben Phänomene hinzu. Es kann davon ausgegangen werden, dass die Immunsuppression prinzipiell als reversibel zu betrachten ist.
Common variable immunodeficiency (CVID) is the most common primary immunodeficiency in adults. It is associated with hypogammaglobulinemia, recurring infections and autoimmune phenomena. Treatment includes immunoglobulin substitution and immunosuppressants. Autoimmune neurological manifestations of CVID are rare and occur predominantly as granulomatous disease. We report the case of a 35-year-old woman with CVID who developed autoimmune encephalitis as demonstrated by double cerebral biopsy. Infectious or malignant causes could be excluded. Despite intensive immunosuppressive therapy with common regimens no significant improvement could be achieved. Ultimately, an autologous hematopoietic stem cell transplantation (HSCT) was performed, resulting in lasting complete remission of the encephalitis. To our knowledge, this is the first report of refractory autoimmune phenomena in CVID treated by autologous HSCT.
Botulinumtoxin A hat in den letzten Jahren als Therapeutikum in den verschiedensten Bereichen der Medizin an Bedeutung dazu gewonnen. In 13 Studien mit insgesamt 320 Kindern und Jugendlichen mit spastischen Bewegungsstörungen haben sich vor allem die Gangstörungen durch Spitzfußstellung, der Adduktorenspasmus und fokale motorische Bewegungsstörungen bei spastischen Bewegungsstörungen als Behandlungsindikationen bei der spastischen Zerebralparese herausgestellt. In der vorliegenden Studie wurden zerebralparetische Kinder im Alter zwischen 1 und 9 Jahren mit einem spastischen Spitzfuß, die mit Botulinumtoxin A behandelt wurden, mit Hilfe eines Auswertebogens für Videosequenzen, blind bezüglich des Behandlungsstatus, evaluiert. Aus den 16 Items wurden 11 zu einem Score addiert. Mit Hilfe dieses Scores konnte ein Therapieerfolg bei 17 von 25 Erstbehandlungen nachgewiesen werden. Die Items 2 (Fußposition in der Standphase), 3 (Fersenkontakt bei Gewichtsübernahme), 6 (Fersenbewegung in der späten Standphase) und 9 (Footclearance in der Mitte der Schwungphase) sprachen besonders gut auf die Therapie an. Damit haben sich in dieser Arbeit, wie in einigen anderen Studien auch, eine Stabilisierung in der Standphase und eine erleichterte Schwungphase jeweils durch Abnahme des Spitzfußes als ein besonderer Nutzen der Therapie mit Botulinumtoxin A herausgestellt. Die Wiederholbarkeit der Therapie mit Botulinumtoxin A zeigt sich in dieser Studie dadurch, dass keine unterschiedlichen Therapieerfolge zwischen erster und zweiter In-jektion nachgewiesen werden konnten. Anhand der zwei in dieser Studie gezeigten Langzeitverläufe ist zusätzlich die Reversibilität der Wirkung gut nachzuvollziehen. Im Gegensatz zu einigen anderen Studien, konnte in der vorliegenden Arbeit ein besse-rer Behandlungserfolg der Patienten mit Hemiparese im Vergleich zu den Patienten mit Diparese nachgewiesen werden. Das kommt sowohl im Score als auch bei den Items 2 (Fußposition in der Standphase) und 6 (Fersenbewegung in der späten Standphase zum Ausdruck. Obwohl sich in der vorliegenden Studie eine Abhängigkeit des Therapieerfolgs vom Alter des Patienten nicht festgestellt werden konnte, ist eine frühe Behandlung (möglichst vor dem 6. Lebensjahr) zu empfehlen, denn ein Einsatz in der frühen Kindheit scheint sich positiv auf das Längenwachstum der Muskulatur und damit protektiv auf die Entstehung von Kontrakturen auszuwirken. Zudem kann bei früher Therapie die größere motorische Lernfähigkeit jüngerer Kinder genutzt werden.
Ein adäquater Impfschutz gehört zu den wichtigsten präventiven Maßnahmen für das Personal im Gesundheitsdienst, inklusive der Medizinstudenten. Einerseits werden die Geimpften vor der Ansteckung geschützt, andererseits wird die Übertragung von impfpräventablen Infektionen durch medizinisches Personal auf Patienten vermieden. Im Rahmen des verpflichtenden Praktikums Impfkurs an der Julius-Maximilians-Universität Würzburg wurde von 2004 bis 2014 der Impfstatus von Medizinstudenten im sechsten Semester erfasst.
Das Ziel dieser Arbeit war es, herauszufinden, welchen Impfschutz Medizinstudenten im sechsten Semester an der Universität Würzburg gegen die impfpräventablen Erkrankungen (IPIE) Tetanus, Diphtherie, Pertussis, Masern, Mumps, Röteln, Varizellen und Hepatitis B aufweisen. Dazu wurden von April 2004 bis September 2014 Daten von insgesamt 1388 Medizinstudenten ausgewertet. Im Einzelnen wurden Daten zur Impfanamnese zu allen genannten IPIE ausgewertet, die Erkrankungsanamnese bei Varizellen und serologische Daten zu den IPIE Röteln, Varizellen und Hepatitis B. Die Besonderheit dieser Arbeit ist die Datenerfassung über einen sehr langen Zeitraum und die Verfügbarkeit von anamnestischen Angaben zum Impfstatus als auch die serologi-schen Befunde aus Blutentnahmen, die im Impfkurs durchgeführt wurden. Ergänzend wurde im WS 2014/15 vor Stattfinden des Impfkurses eine Befragung zum Wissensstand und zur Impfmotivation durchgeführt.
Ein hoher Anteil der Studenten hatte keinen den STIKO-Empfehlungen entsprechenden vollständigen Impfstatus gegen die untersuchten IPIE. Der höchste Anteil lag hier bei Pertussis (76%). Etwa ein Viertel der Studenten konnte keine genaue Aussage machen, ob sie eine frühere Varizellenerkrankung durchgemacht hatten. Etwa 5% der Studenten wiesen zudem einen negativen Serostatus für die untersuchten IPIE auf (außer bei Varizellen). Bei Studenten, die keine Impfungen erhalten hatten, waren zu 13% (Masern) bis 68% (Hepatitis B) keine Antikörper nachweisbar. Studenten, die einen den STIKO-Empfehlungen entsprechenden vollständigen Impfstatus bzw. anamnestisch eine frühere Varizellenerkrankung aufwiesen, hatten zu 81% (Hepatitis B) bis 98% (Varizellen) einen positiven Antikörpertiter. Die Geschlechterunterschiede waren beim Röteln-Impfstatus und Röteln-Serostatus am deutlichsten. 30% der Männer hatten bisher keine Röteln-Impfung erhalten und 12% der Männer wiesen einen negativen Röteln-Serostatus auf.
Die Bereitschaft der Studenten, im Impfkurs eine Nachholimpfung durchführen zu lassen, war sehr hoch. So konnten im Impfkurs bestehende Impflücken geschlossen werden. Die Kosten für die Impfstoffe übernahm die Universität Würzburg.
Des Weiteren konnte durch den Fragebogen zum Wissensstand über Impfprävention und zur Impfmotivation nachgewiesen werden, dass die Studenten nicht ausreichend über IPIE informiert sind. Dies wurde besonders dadurch deutlich, dass die Studenten das Ansteckungsrisiko von Hepatitis B mit 64,4% als hoch oder sehr hoch einschätzten und damit überschätzten und die IPIE Varizellen und Pertussis in ihrem Übertragungsrisiko mit 56,2% und 15,1% zu niedrig einschätzten und damit unterschätzten. Interessant ist, dass die Medizinstudenten in der Befragung das Nebenwirkungsrisiko von Impfungen als gering einschätzten. Die Angst vor Nebenwirkungen scheint kaum eine Rolle bei der Entscheidung zu spielen, ob die Medizinstudenten eine Impfung durchführen lassen oder nicht.
In Zusammenschau der Ergebnisse lässt sich sagen, dass es Nachholbedarf zum Erlangen eines den STIKO-Empfehlungen entsprechenden Impfschutzes für Medizinstudenten gibt. An erster Stelle sind die Studenten selbst für ihren Impfstatus verant-wortlich. An zweiter Stelle sollten jedoch die Universität, an der die Studenten ihr Medizinstudium absolvieren, sowie das Lehrkrankenhaus an welchem die Studenten ihre praktische Ausbildung erhalten, sich für den Impfschutz ihrer Studenten verantwortlich fühlen; zum einen im Sinne des Schutzes ihrer Auszubildenden und Praktikanten, zum anderen im Interesse des Schutzes ihrer Patienten vor der Ansteckung mit impfpräventablen Erkrankungen. Medizinstudenten in die Definition „medizinisches Personal“ ein-zuschließen, kann für einen besseren Anschluss an den Betriebsärztlichen Dienst des Lehrkrankenhauses sorgen. Zudem ist die Erstellung von speziellen Impfempfehlungen für Medizinstudenten sinnvoll um einen adäquaten Schutz gegen IPIE zu erzielen. Die Kostenübernahme für Impfungen für Medizinstudenten muss hierfür geklärt werden. Veranstaltungen, wie der verpflichtende Impfkurs an der Universität Würzburg bieten in der medizinischen Ausbildung die doppelte Chance, den Wissensstand von Medizinstudenten über Impfungen und impfpräventable Infektionserkrankungen zu verbessern und Impflücken zu schließen.
Virale Reaktivierungen treten im Rahmen der Immundefizienz und Immunsuppression nach allogener Stammzelltransplantation häufig auf und können zu schwerwiegenden Komplikationen führen. Ziel dieser retrospektiven Studie war die Charakterisierung von viralen Reaktivierungen im ersten Jahr nach allogener Stammzelltransplantation, die Identifikation von Risikofaktoren sowie die Untersuchung des Einflusses viraler Reaktivierungen auf das Transplantationsoutcome. 107 pädiatrische allogene Stammzelltransplantationen im Zeitrahmen von Januar 2005 bis Dezember 2015 wurden in diesem Zusammenhang auf Infektionen mit dem Epstein-Barr Virus (EBV), Cytomegalovirus (CMV), Humanen Herpesvirus 6 (HHV 6), Herpes simplex Virus (HSV), Varicella zoster Virus (VZV) und Adenovirus (ADV) untersucht.
Obwohl die Geschichte der Kindheit seit Jahrhunderten auch mit Kindesbestrafung, Kindesmisshandlung und sogar Kindstötung verbunden ist, definierten erst Ende der 40er Jahre zunächst J. Caffey und F. N. Silverman, in den 60er Jahren jedoch vor allem H. Kempe und R. E. Helfer das Krankheitsbild des „battered child“ mit den Trias: • radiologisch nachweisbare subperiostale Blutungen mit Verkalkungen, Frakturen und traumatischen Gelenkabsprengungen • Subduralergüssen und • Retinablutungen. Diese Symptome mögen in ihrer Gesamtheit zwar spezifisch für eine traumatische Einwirkung sein, doch ist eine detaillierte, insbesondere radiologische Diagnostik zum Ausschluss anderer Leiden von großer Wichtigkeit. Eine medizinhistorische Aufarbeitung der ersten pädiatrischen Röntgenbilder zwischen 1895 und 1910 zeigt, dass ein damals häufig diagnostiziertes Krankheitsbild die Möller-Barlow Krankheit war, die erstmal von J. Möller im Jahre 1859 beschrieben wurde. Schon damals galt sie überwiegend als Vitamin-C-Mangelerkrankung des Säuglings und Kleinkindes, die sich klinisch vor allem in Form einer hochgradigen Berührungsempfindlichkeit sowie Extremitätenschwellungen äußerte. Dies konnten C. Hart und O. Lessing durch Tierexperimente bestätigten. Dank ihrer, sowie auch J. Schoedels, C. Nauwercks und C. Schmorls vielzähliger makro- und mikroskopischer Studien konnten die charakteristischen Befunde • verkalkte subperiostale Blutungen • metaphysäre Trümmerfeldzonen • zentrale Osteoporose und • Verbreiterung und Auftreibung der Knorpel-Knochen-Grenzen erhoben werden. Schon damals wiesen jedoch bedeutende Kinderärzte wie O. Heubner darauf hin, dass die wirkliche Pathophysiologie erst unzureichend geklärt sei. Seit 1915 wurden kaum weitere Original-Kasuistiken publiziert und heute wird das Krankheitsbild nur noch in Einzelfällen beobachtet. Eine Bewertung der vor 1910 angefertigten Röntgenbilder von Kindern, bei denen M.-B. Krankheit diagnostiziert wurde, lässt keinen einheitlichen Befund erheben und zumindest ein Teil der pathologischen Auffälligkeiten könnte aus heutiger Sicht durchaus als Folge traumatischer Einwirkungen im Sinne eines Schütteltraumas mit subperiostalen Blutungen interpretiert werden. Die Tierexperimente weisen aber auch darauf hin, dass ein exzessiver Vitamin-C-Mangel subperiostale Hämorrhagien entstehen lassen. Des Weiteren lassen die bei der M.-B. Krankheit beschriebenen Röntgenveränderungen auch eine Vielzahl von Differentialdiagnosen zu, wie z. B. die Rachitis, das Menkes-Syndrom, die Lues connata, die Osteogenesis imperfecta, die akute Leukämie und vor allem die Kindesmisshandlung. Resümierend sollte die Erstbeschreibung radiologischer Veränderungen des typischen Schütteltraumas im Säuglings- und Kleinkindesalter um mindestens 50 Jahre vorverlegt und die M.-B. Krankheit vor allem als medizinhistorisches Phänomen differenter Ursache angesehen werden. Radiologische Veränderungen in Form subperiostaler Blutungen, Verkalkungen und einer gestörten Ossifikation sind in erster Linie als Folgen eines Schütteltraumas anzusehen, doch sollte bei der Differentialdiagnostik auch an Ernährungsstörungen mit Vitamin-C-Mangel gedacht werden.
Das grampositive Bakterium Staphylococcus epidermidis ist ein wesentlicher Bestandteil der kommensalen Flora der Haut und der Schleimhäute des Menschen. Jedoch stellen diese Bakterien eine häufige Ursache nosokomialer Katheter-assozierter Infektionen bei immunsupprimierten Patienten dar. Dies liegt zum einen an der Fähigkeit von S. epidermidis, Biofilm zu bilden. Diese physikalische Barriere schützt die Bakterien vor dem Immunsystem sowie vor Antibiotika. Dabei zählen sie zu den häufigsten Erregern von Infektionen an implantierten Fremdkörpern mit Plastikoberflächen, wie z. B. Venenkathetern, künstlichen Herzklappen oder Gefäßprothesen. Zum anderen stellt die Antibiotikaresistenzentwicklung unter S. epidermidis ein zunehmendes Problem dar. Vor allem die late-onset Sepsis, die durch S. epidermidis als Erreger verursacht werden kann, stellt für Frühgeborene eine Gefahr dar. Ziel der vorliegenden Arbeit war, für S. epidermidis als häufigsten und klinisch bedeutsamen KoNS zu eruieren, ob die zunehmende Dauer des stationären Krankenhausaufenthaltes von sehr kleinen Frühgeborenen mit einer höheren Rate an ica-Präsenz, Biofilmbildung und Antibiotikaresistenz assoziiert ist, sowie die Verbreitungswege und das Reservoir für diese S. epidermidis-Stämme zu identifizieren. Hierzu wurden sequenzielle Isolate von S. epidermidis bei Müttern, Kindern und vom Krankenhauspersonal gewonnen und mittels MLST (Multilocus-Sequence-Typing) klonal typisiert. Sie wurden auf Antibiotikaresistenzen, Biofilmbildung und Präsenz des icaA-Gens, das eine Rolle bei der Biofilmbildung spielt, sowie des mecA-Gens untersucht und mit Isolaten, die aus Blutkulturen oder Venenkatheter des Kindes isoliert wurden, verglichen. Es fiel auf, dass die Isolate der sehr kleinen Frühgeborenen deutlich mehr Virulenzfaktoren, wie z.B. Biofilmbildung, hohe Antibiotikaresistenzraten sowie die Präsenz des mecA- und des icaA- Gens, als die maternalen Stämme besaßen. Im Vergleich mit den Ergebnissen der untersuchten Personalstämme liegt der Verdacht nahe, dass oftmals auch das Personal als Transmitter, vor allem von Klonen mit mehreren Virulenzfaktoren, dient. Das Krankenhaus-Milieu scheint dabei ein ideales Reservoir für die Ausbreitung solcher gefährlichen S. epidermidis-Stämme zu sein.
Background
Lipoblastoma is a rare benign mesenchymal neoplasm of infancy that most commonly occurs on the extremities and trunk but can arise at variable sites of the body. Retroperitoneal lipoblastomas are particularly rare but can grow to enormous size, and preoperative diagnosis is difficult with diverse, mostly malignant differential diagnoses that would lead to aggressive therapy. Since lipoblastoma is a benign tumor that has an excellent prognosis after resection, correct diagnosis is crucial.
Case presentation
A case of a large retroperitoneal tumor of a 24-month old infant that was clinically suspicious of a malignant tumor is presented. Due to proximity to the right kidney, clinically most probably a nephroblastoma or clear cell sarcoma of the kidney was suspected. Radiological findings were ambiguous. Therefore, the mass was biopsied, and histology revealed an adipocytic lesion. Although mostly composed of mature adipocytes, in view of the age of the patient, the differential diagnosis of a (maturing) lipoblastoma was raised, which was supported by molecular analysis demonstrating a HAS2-PLAG1 fusion. The tumor was completely resected, and further histopathological workup led to the final diagnosis of a 13 cm large retroperitoneal maturing lipoblastoma. The child recovered promptly from surgery and showed no evidence of recurrence so far.
Conclusion
Although rare, lipoblastoma should be included in the differential diagnoses of retroperitoneal tumors in infants and children, and molecular diagnostic approaches could be a helpful diagnostic adjunct in challenging cases.
Bei Menschen, die an cystischer Fibrose oder auch an einer Hypothyreose erkrankt sind, ist die Beteiligung des Herz-Kreislauf-Systems eine bekannte Komplikation. Ziel dieser Arbeit war, möglicherweise bislang unbekannte Auswirkungen dieser Erkrankungen auf das Belastungs-EKG von Kindern und Jugendlichen zu identifizieren und ggf. zu deuten. Dadurch sollte die Beurteilbarkeit des Belastungs-EKGs bei der Untersuchung der genannten Patientengruppen verbessert werden. Es wurden insgesamt 64 Patienten zwischen 11 und 18 Jahren zu dieser Studie herangezogen, die in drei Gruppen – nämlich gesunde Kontrollen, hypothyreote Patienten und CF-Patienten – aufgeteilt wurden. Sowohl das Ruhe- als auch das Belastungs-EKG dieser Kinder und Jugendlichen wurden in der Extremitäten-Ableitung II bezüglich der verschiedenen EKG-Parameter vermessen und statistisch ausgewertet. Die QT- und PQ-Dauer wurden zur Auswertung herzfrequenzkorrigiert. Bei hypothyreoten Patienten konnte sowohl in Ruhe als auch unter maximaler Belastung eine Bradykardie und eine relative Verlängerung der QT-Dauer festgestellt werden. In Ruhe war eine erniedrigte P-Amplitude zu erkennen. Es konnte gezeigt werden, dass unter körperlicher Belastung zusätzlich eine Verlängerung der P-Dauer eintrat. Im Vergleich zu gesunden Probanden nahm die Dauer der P-Welle unter Belastung bei CF-Patienten weniger deutlich ab. Besonders auffällig war die Amplitude der T-Welle, die im Gegensatz zu den Werten der Kontrollprobanden in Ruhe sehr hoch war und unter Belastung abnahm. Es ist allerdings noch unklar, warum die T-Amplitude bei Mukoviszidose-Patienten auf diese Weise reagiert. Die Hypothese, dass die hohe T-Amplitude in Ruhe bei Mukoviszidose-Patienten auf eine andere Herzachse, d. h. einen anderen Vektor zurückzuführen wäre, ließ sich nicht bestätigen. Eine mögliche Erklärung ist eine Verschiebung des intra- oder parazellulären Elekrolythaushalts, möglicherweise auch durch die genetische Veränderung des CFTR-Proteins. Dies zu klären, müsste Gegenstand weiterer Untersuchungen sein. Diese Studie hat die Bedeutung des Belastungs-EKGs für die Diagnose einer Rechtsherzbelastung nur indirekt durch den Vergleich von EKG-Befunden mit klinischen Daten, nicht jedoch im Vergleich zu bildgebenden Verfahren untersucht. Es ist jedoch bekannt, dass das EKG in seiner diagnostischen Wertigkeit für das Cor pulmonale eingeschränkt ist. Es müssen daher weitere Untersuchungsverfahren wie Herzechographie, Vektorkardiographie, Blutgasanalysen u. a. als zusätzliche Diagnostika eingesetzt werden. Nur durch die Kombination mehrerer diagnostischer Mittel lässt sich mit Sicherheit eine Rechtsherzbeteiligung ermitteln und ein therapeutisches Konzept gegen diese lebensbedrohliche Komplikation der zugrunde liegenden Störung entwickeln. Zusammenfassend hat diese Studie gezeigt, dass das Belastungs-EKG von Patienten mit Hypothyreose und Mukoviszidose gegenüber Gesunden verändert ist. Das EKG kann allerdings nicht als einziges Diagnostikum bei Fragestellungen wie nach einem Cor pulmonale eingesetzt werden.
Gegenstand der Arbeit: Es wurde der Einfluss einer Ausreifung dendritischer Zellen mit Poly(I:C), R848 und Prostaglandin E2 (=tlrDCs) zur Verwendung im Rahmen der Tumorvakzine untersucht. Für den Einsatz einer doppelten TLR-Stimulation gibt es zahlreiche zellphysiologische Gründe, wobei PGE2 als Motilitätsförderer eingesetzt wird. Es besitzt negative Teilwirkungen auf die Zytokinsekretion, eine verbesserte Migration stellt aber die wichtigste Stellgröße zur Optimierung der Tumorvakzine dar.
Ergebnisse: Für tlrDCs konnte neben einer hohen Fähigkeit zu Migration und Kostimulation eine überlegene Immunstimulation für naive CTLs und TH1/TC1-Antworten in einem antigenspezifischen Primingmodell nachgewiesen werden. Eine Ausreifungsdauer von 16 h erscheint für die Zytokinsekretion der DCs günstig. Es lässt sich eine hohe Wahrscheinlichkeit für die Generation von Central-Memory-T-Zellen und das T-Zell-Homing ins ZNS ableiten.
Mature dendritic cells (DCs) represent cellular adjuvants for optimal antigen presentation in cancer vaccines. Recently, a combination of prostaglandin E\(_2\) (PGE\(_2\)) with Toll-like receptor agonists (TLR-P) was proposed as a new standard to generate superior cytokine-producing DCs with high migratory capacity. Here, we compare TLR-P DCs with conventional DCs matured only with the proinflammatory cytokines TNFα and IL-1ß (CDCs), focussing on the interaction of resulting DCs with CD8\(^+\) T-cells. TLR-P matured DCs showed elevated expression of activation markers such as CD80 and CD83 compared to CDCs, together with a significantly higher migration capacity. Secretion of IL-6, IL-8, IL-10, and IL-12 was highest after 16 h in TLR-P DCs, and only TLR-P DCs secreted active IL-12p70. TLR-P DCs as well as CDCs successfully primed multifunctional CD8\(^+\) T-cells from naïve precursors specific for the peptide antigens Melan-A, NLGN4X, and PTP with comparable priming efficacy and T-cell receptor avidity. CD8\(^+\) T-cells primed by TLR-P DCs showed significantly elevated expression of the integrin VLA-4 and a trend for higher T-cell numbers after expansion. In contrast, TLR-P DCs displayed a substantially reduced capability to cross-present CMVpp65 protein antigen to pp65-specific T cells, an effect that was dose-dependent on PGE2 during DC maturation and reproducible with several responder T-cell lines. In conclu-sion, TLR-P matured DCs might be optimal presenters of antigens not requiring processing such as short peptides. However, PGE\(_2\) seems less favorable for maturation of DCs intended to process and cross-present more complex vaccine antigens such as lysates, proteins or long peptides.
Spondyloenchondrodysplasia (SPENCD) is a rare skeletal dysplasia, characterized by metaphyseal lesions, neurological impairment and immune dysregulation associated with lupus-like features. SPENCD is caused by biallelic mutations in the ACP5 gene encoding tartrate-resistant phosphatase. We report on a child, who presented with spasticity, multisystem inflammation, autoimmunity and immunodeficiency with minimal metaphyseal changes due to compound heterozygosity for two novel ACP5 mutations. These findings extend the phenotypic spectrum of SPENCD and indicate that ACP5 mutations can cause severe immune dysregulation and neurological impairment even in the absence of metaphyseal dysplasia.
Background
Surfactant replacement therapy is the standard of care for the prevention and treatment of neonatal respiratory distress syndrome. New generation synthetic surfactants represent a promising alternative to animal-derived surfactants. CHF5633, a new generation reconstituted synthetic surfactant containing SP-B and SP-C analogs and two synthetic phospholipids has demonstrated biophysical effectiveness in vitro and in vivo. While several surfactant preparations have previously been ascribed immunomodulatory capacities, in vitro data on immunomodulation by CHF5633 are limited, so far. Our study aimed to investigate pro- and anti-inflammatory effects of CHF5633 on native and LPS-stimulated human adult monocytes.
Methods
Highly purified adult CD14\(^{+}\) cells, either native or simultaneously stimulated with LPS, were exposed to CHF5633, its components, or poractant alfa (Curosurf\(^{®}\)). Subsequent expression of TNF-α, IL-1β, IL-8 and IL-10 mRNA was quantified by real-time quantitative PCR, corresponding intracellular cytokine synthesis was analyzed by flow cytometry. Potential effects on TLR2 and TLR4 mRNA and protein expression were monitored by qPCR and flow cytometry.
Results
Neither CHF5633 nor any of its components induced inflammation or apoptosis in native adult CD14\(^{+}\) monocytes. Moreover, LPS-induced pro-inflammatory responses were not aggravated by simultaneous exposure of monocytes to CHF5633 or its components. In LPS-stimulated monocytes, exposure to CHF5633 led to a significant decrease in TNF-α mRNA (0.57 ± 0.23-fold, p = 0.043 at 4h; 0.56 ± 0.27-fold, p = 0.042 at 14h). Reduction of LPS-induced IL-1β mRNA expression was not significant (0.73 ± 0.16, p = 0.17 at 4h). LPS-induced IL-8 and IL-10 mRNA and protein expression were unaffected by CHF5633. For all cytokines, the observed CHF5633 effects paralleled a Curosurf®-induced modulation of cytokine response. TLR2 and TLR4 mRNA and protein expression were not affected by CHF5633 and Curosurf®, neither in native nor in LPS-stimulated adult monocytes.
Conclusion
The new generation reconstituted synthetic surfactant CHF5633 was tested for potential immunomodulation on native and LPS-activated adult human monocytes. Our data confirm that CHF5633 does not exert unintended pro-inflammatory effects in both settings. On the contrary, CHF5633 significantly suppressed TNF-α mRNA expression in LPS-stimulated adult monocytes, indicating potential anti-inflammatory effects.
Background: Controversy remains concerning the impact of Ureaplasma on preterm neonatal morbidity.
Methods: Prospective single-center study in very low birth weight infants <30 weeks' gestation. Cord blood and initial nasopharyngeal swabs were screened for Ureaplasma parvum and U. urealyticum using culture technique and polymerase chain reaction. Neonatal outcomes were followed until death or discharge. Multi-analyte immunoassay provided cord blood levels of inflammatory markers. Using multivariate regression analyses, perinatal Ureaplasma exposure was evaluated as risk factor for the development of bronchopulmonary dysplasia (BPD), other neonatal morbidities until discharge and systemic inflammation at admission.
Results: 40/103 (39%) infants were positive for Ureaplasma in one or both specimens, with U. parvum being the predominant species. While exposure to Ureaplasma alone was not associated with BPD, we found an increased risk of BPD in Ureaplasma-positive infants ventilated ≥5 days (OR 1.64; 95% CI 0.12–22.98; p = 0.009). Presence of Ureaplasma was associated with a 7-fold risk of late onset sepsis (LOS) (95% CI 1.80–27.39; p = 0.014). Moreover, Ureaplasma-positive infants had higher I/T ratios (b 0.39; 95% CI 0.08–0.71; p = 0.014), increased levels of interleukin (IL)-17 (b 0.16; 95% CI 0.02–0.30; p = 0.025) and matrix metalloproteinase 8 (b 0.77; 95% CI 0.10–1.44; p = 0.020), decreased levels of IL-10 (b −0.77; 95% CI −1.58 to −0.01; p = 0.043) and increased ratios of Tumor necrosis factor-α, IL-8, and IL-17 to anti-inflammatory IL-10 (p = 0.003, p = 0.012, p < 0.001).
Conclusions: Positive Ureaplasma screening was not associated with BPD. However, exposure contributed to BPD in infants ventilated ≥5 days and conferred an increased risk of LOS and imbalanced inflammatory cytokine responses.
Developmentally regulated features of innate immunity are thought to place preterm and term infants at risk of infection and inflammation-related morbidity. Underlying mechanisms are incompletely understood. Differences in monocyte function including toll-like receptor (TLR) expression and signaling have been discussed. Some studies point to generally impaired TLR signaling, others to differences in individual pathways. In the present study, we assessed mRNA and protein expression of pro- and anti-inflammatory cytokines in preterm and term cord blood (CB) monocytes compared with adult controls stimulated ex vivo with Pam3CSK4, zymosan, polyinosinic:polycytidylic acid, lipopolysaccharide, flagellin, and CpG oligonucleotide, which activate the TLR1/2, TLR2/6, TLR3, TLR4, TLR5, and TLR9 pathways, respectively. In parallel, frequencies of monocyte subsets, stimulus-driven TLR expression, and phosphorylation of TLR-associated signaling molecules were analyzed. Independent of stimulus, pro-inflammatory responses of term CB monocytes equaled adult controls. The same held true for preterm CB monocytes—except for lower IL-1β levels. In contrast, CB monocytes released lower amounts of anti-inflammatory IL-10 and IL-1ra, resulting in higher ratios of pro-inflammatory to anti-inflammatory cytokines. Phosphorylation of p65, p38, and ERK1/2 correlated with adult controls. However, stimulated CB samples stood out with higher frequencies of intermediate monocytes (CD14\(^+\)CD16\(^+\)). Both pro-inflammatory net effect and expansion of the intermediate subset were most pronounced upon stimulation with Pam3CSK4 (TLR1/2), zymosan (TR2/6), and lipopolysaccharide (TLR4). Our data demonstrate robust pro-inflammatory and yet attenuated anti-inflammatory responses in preterm and term CB monocytes, along with imbalanced cytokine ratios. Intermediate monocytes, a subset ascribed pro-inflammatory features, might participate in this inflammatory state.
Background:
Ureaplasma species have been associated with chorioamnionitis and preterm birth and have been implicated in the pathogenesis of neonatal short and long-term morbidity. However, being mostly commensal bacteria, controversy remains on the pro-inflammatory capacity of Ureaplasma. Discussions are ongoing on the incidence and impact of prenatal, perinatal, and postnatal infection. The present study addressed the impact of Ureaplasma isolates on monocyte-driven inflammation.
Methods:
Cord blood monocytes of term neonates and adult monocytes, either native or LPS-primed, were cultured with Ureaplasma urealyticum (U. urealyticum) serovar 8 (Uu8) and Ureaplasma parvum serovar 3 (Up3). Using qRT-PCR, cytokine flow cytometry, and multi-analyte immunoassay, we assessed mRNA and protein expression of tumor necrosis factor (TNF)-α, interleukin (IL)-1β, IL-8, IL-12p40, IL-10, and IL-1 receptor antagonist (IL-1ra) as well as Toll-like receptor (TLR) 2 and TLR4.
Results:
Uu8 and Up3 induced mRNA expression and protein release of TNF-α, IL-1β and IL-8 in term neonatal and adult monocytes (p < 0.01 and p < 0.05). Intracellular protein expression of TNF-α, IL-1β and IL-8 in Ureaplasma-stimulated cells paralleled those results. Ureaplasma-induced cytokine levels did not significantly differ from LPS-mediated levels except for lower intracellular IL-1β in adult monocytes (Uu8: p < 0.05). Remarkably, ureaplasmas did not induce IL-12p40 response and promoted lower amounts of anti-inflammatory IL-10 and IL-1ra than LPS, provoking a cytokine imbalance more in favor of pro-inflammation (IL-1β/IL-10, IL-8/IL-10 and IL-8/IL-1ra: p < 0.01, vs. LPS). In contrast to LPS, both isolates induced TLR2 mRNA in neonatal and adult cells (p < 0.001 and p < 0.05) and suppressed TLR4 mRNA in adult monocytes (p < 0.05). Upon co-stimulation, Uu8 and Up3 inhibited LPS-induced intracellular IL-1β (p < 0.001 and p < 0.05) and IL-8 in adult monocytes (p < 0.01), while LPS-induced neonatal cytokines were maintained or aggravated (p < 0.05).
Conclusion:
Our data demonstrate a considerable pro-inflammatory capacity of Ureaplasma isolates in human monocytes. Stimulating pro-inflammatory cytokine responses while hardly inducing immunomodulatory and anti-inflammatory cytokines, ureaplasmas might push monocyte immune responses toward pro-inflammation. Inhibition of LPS-induced cytokines in adult monocytes in contrast to sustained inflammation in term neonatal monocytes indicates a differential modulation of host immune responses to a second stimulus. Modification of TLR2 and TLR4 expression may shape host susceptibility to inflammation.
Within the last decades, therapeutic advances, such as antenatal corticosteroids, surfactant replacement, monitored administration of supplemental oxygen, and sophisticated ventilatory support have significantly improved the survival of extremely premature infants. In contrast, the incidence of some neonatal morbidities has not declined. Rates of bronchopulmonary dysplasia (BPD) remain high and have prompted neonatologists to seek effective strategies of non-invasive respiratory support in high risk infants in order to avoid harmful effects associated with invasive mechanical ventilation. There has been a stepwise replacement of invasive mechanical ventilation by early continuous positive airway pressure (CPAP) as the preferred strategy for initial stabilization and for early respiratory support of the premature infant and management of respiratory distress syndrome. However, the vast majority of high risk babies are mechanically ventilated at least once during their NICU stay. Adjunctive therapies aiming at the prevention of CPAP failure and the support of functional residual capacity have been introduced into clinical practice, including alternative techniques of administering surfactant as well as non-invasive ventilation approaches. In contrast, the strategy of applying sustained lung inflations in the delivery room has recently been abandoned due to evidence of higher rates of death within the first 48 h of life.
Regulatorische T-Lymphozyten differenzieren sich im fetalen Thymus unter dem Einfluss des Transkriptionsfaktors FoxP3. Sie sind für die Aufrechterhaltung des Gleichgewichts des Immunsystems wichtig. Es wurde untersucht ob eine Chorioamnionitis, induziert durch intraamniotische Endotoxingabe, die fetale Thymusentwicklung beeinflusst. Den Mutterschafen wurde fünf Tage, zwei Tage, einen Tag oder fünf Stunden vor der Sectio cesarea 10mg Endotoxin intraamniotisch verabreicht. Die Sectio cesarea wurde bei einem Gestationsalter von 123 Tagen durchgeführt. Der entnommene Thymus wurde gewogen, Nabelschnurblutlymphozyten und Plamakortisolwerte wurden bestimmt. Glukokortikoidrezeptoren, aktivierte Caspase-3-, Ki67-, PCNA-, NFkB- und FoxP3-positive Zellen wurden immunohistochemisch nachgewiesen. Das Thymusgewicht war im Verhältnis zum Körpergewicht der Lämmer nach intraamniotischer Endotoxingabe zu allen gemessenen Zeitpunkten verringert. Die zirkulierenden Lymphozyten im Nabelschnurblut nahmen einen Tag nach Endotoxingabe um 40% ab. Die Endotoxingabe führte zu einem vorübergehenden Anstieg der Plasmakortisolwerte, zu einer Verdoppelung NFkB positiver Zellen und zu einer Abnahme Foxp3 positiver Zellen in der Thymusrinde einen Tag nach Endotoxingabe. Die intraamniotische Verabreichung eines Endotoxins führte im Schafmodell zu Veränderungen im fetalen Thymus.
Introduction
Detailed and up-to-date data on the epidemiology and healthcare costs of Influenza are fundamental for public health decision-making. We analyzed inpatient data on Influenza-associated hospitalizations (IAH), selected complications and risk factors, and their related direct costs for Germany during ten consecutive years.
Methods
We conducted a retrospective cost-of-illness study on patients with laboratory-confirmed IAH (ICD-10-GM code J09/J10 as primary diagnosis) by ICD-10-GM-based remote data query using the Hospital Statistics database of the German Federal Statistical Office. Clinical data and associated direct costs of hospital treatment are presented stratified by demographic and clinical variables.
Results
Between January 2010 to December 2019, 156,097 persons were hospitalized due to laboratory-confirmed Influenza (J09/J10 primary diagnosis). The annual cumulative incidence was low in 2010, 2012 and 2014 (1.3 to 3.1 hospitalizations per 100,000 persons) and high in 2013 and 2015-2019 (12.6 to 60.3). Overall direct per patient hospitalization costs were mean (SD) 3521 EUR (± 8896) and median (IQR) 1805 EUR (1502; 2694), with the highest mean costs in 2010 (mean 8965 EUR ± 26,538) and the lowest costs in 2012 (mean 2588 EUR ± 6153). Mean costs were highest in 60-69 year olds, and in 50-59, 70-79 and 40-49 year olds; they were lowest in 10-19 year olds. Increased costs were associated with conditions such as diabetes (frequency 15.0%; 3.45-fold increase compared to those without diabetes), adiposity (3.3%; 2.09-fold increase) or immune disorders (5.6%; 1.88-fold increase) and with Influenza-associated complications such as Influenza pneumonia (24.3%; 1.95-fold), bacterial pneumonia (6.3%; 3.86-fold), ARDS (1.2%; 10.90-fold increase) or sepsis (2.3%; 8.30-fold). Estimated overall costs reported for the 10-year period were 549.6 Million euros (95% CI 542.7-556.4 million euros).
Conclusion
We found that the economic burden of IAH in Germany is substantial, even when considering solely laboratory-confirmed IAH reported as primary diagnosis. The highest costs were found in the elderly, patients with certain underlying risk factors and patients who required advanced life support treatment, and median and mean costs showed considerable variations between single years. Furthermore, there was a relevant burden of disease in middle-aged adults, who are not covered by the current vaccination recommendations in Germany.
Die Dissertation untersucht hospitalisierte Patienten in der Kinderklinik des Universitätsklinikum Innsbruck, die wegen einer rotaviren assoziierten Gastroenteritis hospitalisiert wurden. Der Untersuchungszeitraum war von Januar 2002 bis Dezmber 2012.Untersucht wurde die Hospitalisierungsrate, Hospitalisierungsdauer, Hospitalisierungsalter, die Schwere der Erkrankung sowie eine Änderung von nosokomialen und ambulanten Erwerb vor und nach Impfeinführung im Juli 2007.
Beeinflussung der Aktivität von NF-kappaB durch gram-negative Bakterien - ein Beitrag zur Pathogenese der reaktiven Arthritis Als Beitrag zur Pathogenese der reaktiven Arthritis wurde die Aktivierung von NF-kB durch gram-negative Bakterien untersucht. Mittelpunkt dieser Arbeit war die Beobachtung, dass die Aktivität des Transkriptionsfaktors NF-kB in Synovialzellen nach Infektion stimuliert wird. Die Erkrankung steht im klinischen Zusammenhang mit einer Infektion durch gram-negative Darmbakterien und weiteren Erregern. TNF-a spielt eine wichtige Rolle bei der Erregerantwort der infizierten Zellen, in welchen erhöhte TNF-a-Titer gemessen wurden. Das bekannte Mitwirken von NF-kB in immunologischen Prozessen ließ vermuten, dass dieser Transkriptionsfaktor an der Pathogenese der reaktiven Arthritis beteiligt ist. Dies steht in engem Zusammenhang mit der Induktion von TNF-a, ein Zytokin, das gleichzeitig ein wichtiger Induktor von NF-kB darstellt. In unseren Experimenten wurde ein Unterschied zwischen apathogenen und pathogenen Keimen in der zeitlichen Aktivierung von NF-kB beobachtet. Die Vertreter pathogener Erreger waren Yersinia enterocolitica O.3 und Salmonella enteritidis. Diese induzierten NF-kB zwischen 4 und 6 Stunden post infectionem, im Unterschied zu dem apathogenen Bakterium Escherichia coli, das den Transkriptionsfaktor schon nach 1 - 2 Stunden induzierte. Als Folge dieser Differenz könnte die Immunantwort der Zelle zu unterschiedlichen Reaktionen in der Lage sein und die Erreger abtöten oder eine Persistenz zulassen. Zusätzlich wurde das Augenmerk auf die einzelnen Zellbestandteile oder –produkte gelenkt. Im Vergleich zu intakten Bakterien wurde die Wirkung des Überstandes und die von UV-inaktivierten Keimen untersucht. Die Induktionsstärke war bei unbehandelten Erregern am größten und fiel dann bei UV-inaktivierten Bakterien deutlich ab. Ein weiteres Abfallen der Aktivierung war bei der Infektion mit dem bloßen Überstand zu verzeichnen. Mit diesen Ergebnissen wurde deutlich, dass NF-kB bei der Etablierung der reaktiven Arthritis eine Rolle spielen könnte. Noch bleibt offen, in welcher Art der Transkriptionsfaktor in die intrazellulären Prozesse eingreift und welche medikamentösen Behandlungsmöglichkeiten sich daraus ergeben könnten.
Immune checkpoint blockade therapy is beneficial and even curative for some cancer patients. However, the majority don’t respond to immune therapy. Across different tumor types, pre-existing T cell infiltrates predict response to checkpoint-based immunotherapy. Based on in vitro pharmacological studies, mouse models and analyses of human melanoma patients, we show that the cytokine GDF-15 impairs LFA-1/β2-integrin-mediated adhesion of T cells to activated endothelial cells, which is a pre-requisite of T cell extravasation. In melanoma patients, GDF-15 serum levels strongly correlate with failure of PD-1-based immune checkpoint blockade therapy. Neutralization of GDF-15 improves both T cell trafficking and therapy efficiency in murine tumor models. Thus GDF-15, beside its known role in cancer-related anorexia and cachexia, emerges as a regulator of T cell extravasation into the tumor microenvironment, which provides an even stronger rationale for therapeutic anti-GDF-15 antibody development.
Chemokines (C-X-C) motif ligand (CXCL) 5 and 8 are overexpressed in patients with multiple sclerosis, where CXCL5 serum levels were shown to correlate with blood–brain barrier dysfunction as evidenced by gadolinium-enhanced magnetic resonance imaging. Here, we studied the potential role of CXCL5/CXCL8 receptor 2 (CXCR2) as a regulator of paraendothelial brain barrier function, using the well-characterized human cerebral microvascular endothelial cell line hCMEC/D3. Low basal CXCR2 mRNA and protein expression levels in hCMEC/D3 were found to strongly increase under inflammatory conditions. Correspondingly, immunohistochemistry of brain biopsies from two patients with active multiple sclerosis revealed upregulation of endothelial CXCR2 compared to healthy control tissue. Recombinant CXCL5 or CXCL8 rapidly and transiently activated Akt/protein kinase B in hCMEC/D3. This was followed by a redistribution of tight junction-associated protein zonula occludens-1 (ZO-1) and by the formation of actin stress fibers. Functionally, these morphological changes corresponded to a decrease of paracellular barrier function, as measured by a real-time electrical impedance-sensing system. Importantly, preincubation with the selective CXCR2 antagonist SB332235 partially prevented chemokine-induced disturbance of both tight junction morphology and function. We conclude that human brain endothelial CXCR2 may contribute to blood–brain barrier disturbance under inflammatory conditions with increased CXCL5 and CXCL8 expression, where CXCR2 may also represent a novel pharmacological target for blood–brain barrier stabilization.
In large vessel occlusion stroke, recanalization to restore cerebral perfusion is essential but not necessarily sufficient for a favorable outcome. Paradoxically, in some patients, reperfusion carries the risk of increased tissue damage and cerebral hemorrhage. Experimental and clinical data suggest that endothelial cells, representing the interface for detrimental platelet and leukocyte responses, likely play a crucial role in the phenomenon referred to as ischemia/reperfusion (I/R)-injury, but the mechanisms are unknown. We aimed to determine the role of endoglin in cerebral I/R-injury; endoglin is a membrane-bound protein abundantly expressed by endothelial cells that has previously been shown to be involved in the maintenance of vascular homeostasis. We investigated the expression of membranous endoglin (using Western blotting and RT-PCR) and the generation of soluble endoglin (using an enzyme-linked immunosorbent assay of cell culture supernatants) after hypoxia and subsequent reoxygenation in human non-immortalized brain endothelial cells. To validate these in vitro data, we additionally examined endoglin expression in an intraluminal monofilament model of permanent and transient middle cerebral artery occlusion in mice. Subsequently, the effects of recombinant human soluble endoglin were assessed by label-free impedance-based measurement of endothelial monolayer integrity (using the xCELLigence DP system) and immunocytochemistry. Endoglin expression is highly inducible by hypoxia in human brain endothelial monolayers in vitro, and subsequent reoxygenation induced its shedding. These findings were corroborated in mice during MCAO; an upregulation of endoglin was displayed in the infarcted hemispheres under occlusion, whereas endoglin expression was significantly diminished after transient MCAO, which is indicative of shedding. Of note is the finding that soluble endoglin induced an inflammatory phenotype in endothelial monolayers. The treatment of HBMEC with endoglin resulted in a decrease in transendothelial resistance and the downregulation of VE-cadherin. Our data establish a novel mechanism in which hypoxia triggers the initial endothelial upregulation of endoglin and subsequent reoxygenation triggers its release as a vasoactive mediator that, when rinsed into adjacent vascular beds after recanalization, can contribute to cerebral reperfusion injury.
In Deutschland treten bei Kindern ca. 400 Neuerkrankungen an Hirntumoren pro Jahr auf. Den größten Anteil an dieser sehr heterogenen Krankheitsgruppe haben benigne niedriggradige Gliome und maligne Medulloblastome. Der Rolle des Immunsystems bei Krebserkrankungen ist schon seit über 100 Jahren Thema der Forschung. Im Zentrum dieser Studie stand der Einfluss des Immunssystems und insbesondere der Cytokine IL-4, IL-6, IL-10, TNF-α, TGF-β und IFN-γ auf die Entwicklung und Prognose bei Kindern mit Hirntumoren. Dabei wurden die jeweiligen Cytokin Gen-Polymorphismen bei 175 Hirntumorpatienten mittels PCR bestimmt. Diese Daten wurden mit den klinischen Parametern der Patienten korreliert und so der Einfluss der jeweiligen Genotypen und Phänotypen auf die Prognose der Erkrankungen untersucht. In einem weiteren Schritt erfolgte eine Analyse der direkten Ausschüttung von Cytokinen durch T-Lymphozyten nach In-vitro Stimulation der Zellen. Die T-Lymphozyten wurden aus dem extrahierten Blut der Studienteilnehmer isoliert und nach einem festen Schema stimuliert. In der Durchflusszytologie wurde dann die Menge an Cytokin-positiven T-Lymphozyten für die entsprechenden Cytokine gemessen und mit einer altersangepassten gesunden Vergleichspopulation verglichen. Als Nullhypothesen wurde definiert, dass in der Studienpopulation die Genotypen und die Phänotypen bei den Cytokin Gen-Polymorphismen und die Menge an Cytokin-positiven T-Lymphozyten durch Stimulation sich nicht signifikant von den entsprechenden Vergleichskohorten unterscheiden. Beide Nullhypothesen können abgelehnt werden. Es gibt, insbesondere bei der direkten In-vitro Stimulation, teils hochsignifikante Unterschiede. Das adaptierte Immunsystem nimmt also auch bei pädiatrischen Tumorerkrankungen des zentralen Nervensystems eine wichtige Funktion ein. Cytokine spielen hierbei durchaus vielfältige und mitunter sogar entgegengesetzte Rollen, wie durch die vorliegenden Untersuchungen gezeigt werden konnten. So hat eine verstärkte Ausschüttung von TGF-β einen protektiven Effekt. Im späteren Verlauf wird durch die immunsuppressive Wirkung des TGF-β wiederum eine adäquate antitumorale Immunreaktion verhindert und so ein Progress der jeweiligen Tumorentitäten begünstigt. Dies kann auch erklären, weswegen eine einfache Hemmung der Cytokine bisher nicht die gewünschten therapeutischen Resultate erbrachte. Aufgrund ihrer enormen Bedeutung für die Generierung einer Immunantwort sind Cytokine eine wichtige Stellschraube in onkologischen Erkrankungen und deren Verlauf. Eine weitere differenzierende Untersuchung der einzelnen Cytokine, aber auch die funktionierende Gesamtheit der Cytokine und des Immunsystems, ist wichtig für die Entwicklung hochwertiger und effektiver immunologischer Therapieansätze.
Die Invasive Aspergillose (IA) stellt eine Hauptursache der infektassoziierten Morbidität und Mortalität bei pädiatrischen Patienten mit hämato-onkologischer Grunderkrankung und/oder allogener Stammzelltransplantation dar. Die sichere und frühzeitige Diagnose ist bei Kindern aufgrund spärlicher pädiatrischer Daten weiterhin eine klinische Herausforderung. Die Kombination der Biomarker Galactomannanantigen und Aspergillus DNA hat sich in Erwachsenenstudien als vorteilhaft in der Diagnose der IA erwiesen. Ziel der durchgeführten Studie war daher, die diagnostische Güte des kombinierten Biomarkerscreenings in einer pädiatrischen Hochrisikokohorte zu ermitteln.
Hierfür wurden 39 pädiatrische Patienten, die während eines Zeitraumes von drei Jahren aufgrund einer hämato-onkologischen Grunderkrankung und Notwendigkeit einer Stammzelltransplantation in der Würzburger Kinderklinik behandelt wurden, einem hochstandardisierten, zweimal wöchentlichen Screening auf Galactomannanantigen und fungaler DNA zugeführt. Zusätzlich wurde für jeden Patienten ein breites Spektrum an klinischen Daten sowie mikrobiologischen und radiologischen Ergebnissen erfasst und die IA-Klassifikation nach den EORTC/MSG-Kriterien durchgeführt.
Unsere Daten zeigten eine IA-Inzidenz (probable IA) von 10%, was per definitionem einer Hochrisikokohorte entspricht. Das kombinierte Monitoring der Biomarker Galactomannanantigen und Aspergillus-DNA wies eine hohe diagnostische Genauigkeit mit einer Sensitivität/Spezifität/PPV/NPV von 1.00 und gute Eignung als Screeningtest auf. Die antifungale Prophylaxe zeigte keinen negativen Einfluss auf die diagnostischen Gütekriterien der beiden Biomarker, wie in anderen Studien postuliert. Der Galactomannanindex erwies sich als vielversprechender Surrogatmarker für das Outcome und das Therapieansprechen.
Weiterführende Studien sind notwendig, um festzulegen, ob die Biomarkerkombination eine Detektion asymptomatischer subklinischer Infektionen als eine Art „Frühwarnsystem“ ermöglicht und somit eine Reduktion der Mortalität bedingen kann.
Circulating MACC1 transcripts in glioblastoma patients predict prognosis and treatment response

Glioblastoma multiforme is the most aggressive primary brain tumor of adults, but lacksreliable and liquid biomarkers. We evaluated circulating plasma transcripts of metastasis-associatedin colon cancer-1 (MACC1), a prognostic biomarker for solid cancer entities, for prediction of clinicaloutcome and therapy response in glioblastomas. MACC1 transcripts were significantly higher inpatients compared to controls. Low MACC1 levels clustered together with other prognosticallyfavorable markers. It was associated with patients’ prognosis in conjunction with the isocitratedehydrogenase (IDH) mutation status: IDH1 R132H mutation and low MACC1 was most favorable(median overall survival (OS) not yet reached), IDH1 wildtype and high MACC1 was worst (medianOS 8.1 months), while IDH1 wildtype and low MACC1 was intermediate (median OS 9.1 months).No patients displayed IDH1 R132H mutation and high MACC1. Patients with low MACC1 levelsreceiving standard therapy survived longer (median OS 22.6 months) than patients with high MACC1levels (median OS 8.1 months). Patients not receiving the standard regimen showed the worstprognosis, independent of MACC1 levels (low: 6.8 months, high: 4.4 months). Addition of circulatingMACC1 transcript levels to the existing prognostic workup may improve the accuracy of outcomeprediction and help define more precise risk categories of glioblastoma patients.
Background:
In 2004, routine varicella vaccination was introduced in Germany for children aged 11–14 months. Routine measles vaccination had already been introduced in 1973 for the same age group, but coverage is still too low (<95%) in some areas to eliminate measles. The present study assessed varicella and measles vaccination coverage and determinants of parental acceptance in two study regions, situated in Northern and Southern Bavaria (Germany).
Methods:
From 2009 to 2011, annual cross-sectional parent surveys were performed on random samples of 600 children aged 18–36 months in the Bavarian regions of both Munich and Würzburg. Logistic regression models were used to identify factors associated with varicella and measles vaccination.
Results:
In 2009, 2010 and 2011, vaccination coverage was lower in Munich than in Würzburg, for both varicella (Munich 53%, 67%, 69% vs. Würzburg 72%, 81%, 83%) and for measles (Munich 88%, 89%, 91% vs. Würzburg 92%, 93%, 95%). Recommendation by the physician was the main independent factor associated with varicella vaccination in both regions (adjusted odd ratios (OR) with 95% confidence interval (CI): Munich OR 19.7, CI 13.6–28.6; Würzburg OR 34.7, CI 22.6–53.2). Attendance at a childcare unit was positively associated with a higher acceptance of varicella vaccination in Munich (OR 1.5, CI 1.1–2.2). Regarding measles vaccination, attendance at a childcare unit was positively associated in both regions (Munich OR 2.0; CI 1.3–3.0; Würzburg OR 1.8; CI 1.1–3.1), and a higher level of parental school education was negatively associated in Würzburg (OR 0.5, CI 0.3–0.9).
Conclusions:
Vaccination rates differed between regions, with rates constantly higher in Würzburg. Within each region, vaccination rates were lower for varicella than for measles. Measles vaccination status was mainly dependent upon socio-demographic factors (attendance at a childcare unit, parental school education), whereas for the more recently introduced varicella vaccination recommendation by the physician had the strongest impact. Hence, different strategies are needed to further improve vaccination rates for both diseases.
This evidence‐based clinical guideline provides consensus‐recommendations for the treatment and care of patients with primary antibody deficiencies (PADs). The guideline group comprised 20 clinical and scientific expert associations of the German, Swiss, and Austrian healthcare system and representatives of patients. Recommendations were based on results of a systematic literature search, data extraction, and evaluation of methodology and study quality in combination with the clinical expertise of the respective representatives. Consensus‐based recommendations were determined via nominal group technique. PADs are the largest clinically relevant group of primary immunodeficiencies. Most patients with PADs present with increased susceptibility to infections, however immune dysregulation, autoimmunity, and cancer affect a significant number of patients and may precede infections. This guideline therefore covers interdisciplinary clinical and therapeutic aspects of infectious (e.g., antibiotic prophylaxis, management of bronchiectasis) and non‐infectious manifestations (e.g., management of granulomatous disease, immune cytopenia). PADs are grouped into disease entities with definitive, probable, possible, or unlikely benefit of IgG‐replacement therapy. Summary and consensus‐recommendations are provided for treatment indication, dosing, routes of administration, and adverse events of IgG‐replacement therapy. Special aspects of concomitant impaired T‐cell function are highlighted as well as clinical data on selected monogenetic inborn errors of immunity formerly classified into PADs (APDS, CTLA‐4‐, and LRBA‐deficiency).
Aim: It was the aim of our study to determine the regional cerebral tissue oxygenation saturation (rcSO\(_2\)) as an additional monitoring parameter during early skin-to-skin contact (SSC) in preterm infants with a gestational age of <32 gestational weeks. Methods: We conducted two observational convenience sample studies using additional monitoring with near-infrared spectroscopy (NIRS) in the first 120 h of life: (a) NIRS 1 (gestational age of 26 0/7 to 31 6/7 weeks) and (b) NIRS 2 (gestational age of 24 0/7 to 28 6/7 weeks). The rcSO\(_2\) values were compared between resting time in the incubator (period I), SSC (period II) and handling nursing care (period III). For the comparison, we separated the sequential effects by including a “wash-out phase” of 1 h between each period. Results: During the first 120 h of life 38/53 infants in NIRS 1 and 15/23 infants in NIRS 2 received SSC, respectively. We found no remarkable differences for rcSO\(_2\) values of NIRS 1 patients between SSC time and period I (95% confidence interval (CI) for the difference in %: SSC vs. period I [1; 3]). In NIRS 2, rcSO\(_2\) values during SSC were only 2% lower compared with period I [median [1. quartile; 3. quartile] in %; 78 [73; 82] vs. 80 [74; 85]] but were similar to period III [78 [72; 83]]. In a combined analysis, a small difference in rcSO\(_2\) values between SSC and resting times was found using a generalized linear mixed model that included gender and gestational age (OR 95% CI; 1.178 [1.103; 1.253], p < 0.0001). Episodes below the cut-off for “hypoxia”; e.g., <55%, were comparable during SSC and periods I and III (0.3–2.1%). No FiO\(_2\) adjustment was required in the vast majority of SSC episodes. Conclusions: Our observational data indicate that rcSO\(_2\) values of infants during SSC were comparable to rcSO\(_2\) values during incubator care and resting time. This additional monitoring supports a safe implementation of early SSC in extremely preterm infants in NICUs.
Background
Physical activity (PA) and exercise have become an accepted and valued component of cystic fibrosis (CF) care. Regular PA and exercise can positively impact pulmonary function, improve physical fitness, and enhance health-related quality of life (HRQoL). However, motivating people to be more active is challenging. Supervised exercise programs are expensive and labour intensive, and adherence falls off significantly once supervision ends. Unsupervised or partially supervised programs are less costly and more flexible, but compliance can be more problematic. The primary objective of this study is to evaluate the effects of a partially supervised exercise intervention along with regular motivation on forced expiratory volume in 1 s (FEV1) at 6 months in a large international group of CF patients. Secondary endpoints include patient reported HRQoL, as well as levels of anxiety and depression, and control of blood sugar.
Methods/design
It is planned that a total of 292 patients with CF 12 years and older with a FEV1 ≥ 35% predicted shall be randomised. Following baseline assessments (2 visits) patients are randomised into an intervention and a control group. Thereafter, they will be seen every 3 months for assessments in their centre for one year (4 follow-up visits). Along with individual counselling to increase vigorous PA by at least 3 h per week on each clinic visit, the intervention group documents daily PA and inactivity time and receives a step counter to record their progress within a web-based diary. They also receive monthly phone calls from the study staff during the first 6 months of the study. After 6 months, they continue with the step counter and web-based programme for a further 6 months. The control group receives standard care and keeps their PA level constant during the study period. Thereafter, they receive the intervention as well.
Discussion
This is the first large, international multi-centre study to investigate the effects of a PA intervention in CF with motivational feedback on several health outcomes using modern technology. Should this relatively simple programme prove successful, it will be made available on a wider scale internationally.

Background
Health-related and disease-specific quality of life (HRQoL) has been increasingly valued as relevant clinical parameter in cystic fibrosis (CF) clinical care and clinical trials. HRQoL measures should assess – among other domains – daily functioning from a patient’s perspective. However, validation studies for the most frequently used HRQoL questionnaire in CF, the Cystic Fibrosis Questionnaire (CFQ), have not included measures of physical activity or fitness. The objective of this study was, therefore, to determine the cross-sectional and longitudinal relationships between HRQoL, physical activity and fitness in patients with CF.
Methods
Baseline (n = 76) and 6-month follow-up data (n = 70) from patients with CF (age ≥12 years, FEV1 ≥35%) were analysed. Patients participated in two multi-centre exercise intervention studies with identical assessment methodology. Outcome variables included HRQoL (German revised multi-dimensional disease-specific CFQ (CFQ-R)), body composition, pulmonary function, physical activity, short-term muscle power, and aerobic fitness by peak oxygen uptake and aerobic power.
Results
Peak oxygen uptake was positively related to 7 of 13 HRQoL scales cross-sectionally (r = 0.30-0.46). Muscle power (r = 0.25-0.32) and peak aerobic power (r = 0.24-0.35) were positively related to 4 scales each, and reported physical activity to 1 scale (r = 0.29). Changes in HRQoL-scores were directly and significantly related to changes in reported activity (r = 0.35-0.39), peak aerobic power (r = 0.31-0.34), and peak oxygen uptake (r = 0.26-0.37) in 3 scales each. Established associates of HRQoL such as FEV1 or body mass index correlated positively with fewer scales (all 0.24 < r < 0.55).
Conclusions
HRQoL was associated with physical fitness, especially aerobic fitness, and to a lesser extent with reported physical activity. These findings underline the importance of physical fitness for HRQoL in CF and provide an additional rationale for exercise testing in this population.
Background
In individuals suffering from a rare disease the diagnostic process and the confirmation of a final diagnosis often extends over many years. Factors contributing to delayed diagnosis include health care professionals' limited knowledge of rare diseases and frequent (co-)occurrence of mental disorders that may complicate and delay the diagnostic process. The ZSE-DUO study aims to assess the benefits of a combination of a physician focusing on somatic aspects with a mental health expert working side by side as a tandem in the diagnostic process.
Study design
This multi-center, prospective controlled study has a two-phase cohort design.
Methods
Two cohorts of 682 patients each are sequentially recruited from 11 university-based German Centers for Rare Diseases (CRD): the standard care cohort (control, somatic expertise only) and the innovative care cohort (experimental, combined somatic and mental health expertise). Individuals aged 12 years and older presenting with symptoms and signs which are not explained by current diagnoses will be included. Data will be collected prior to the first visit to the CRD’s outpatient clinic (T0), at the first visit (T1) and 12 months thereafter (T2).
Outcomes
Primary outcome is the percentage of patients with one or more confirmed diagnoses covering the symptomatic spectrum presented. Sample size is calculated to detect a 10 percent increase from 30% in standard care to 40% in the innovative dual expert cohort. Secondary outcomes are (a) time to diagnosis/diagnoses explaining the symptomatology; (b) proportion of patients successfully referred from CRD to standard care; (c) costs of diagnosis including incremental cost effectiveness ratios; (d) predictive value of screening instruments administered at T0 to identify patients with mental disorders; (e) patients’ quality of life and evaluation of care; and f) physicians’ satisfaction with the innovative care approach.
Conclusions
This is the first multi-center study to investigate the effects of a mental health specialist working in tandem with a somatic expert physician in CRDs. If this innovative approach proves successful, it will be made available on a larger scale nationally and promoted internationally. In the best case, ZSE-DUO can significantly shorten the time to diagnosis for a suspected rare disease.

Purpose of Review
To describe in detail the clinical synopsis and pathophysiology of chronic non-bacterial osteomyelitis and SAPHO syndrome.
Recent Findings
Chronic non-bacterial osteomyelitis (CNO) has been identified as a disease entity for almost 50 years. This inflammatory bone disorder is characterized by osteolytic as well as hyperostotic/osteosclerotic lesions. It is chronic in nature, but it can present with episodic flairs and phases of remission, which have led to the denomination “chronic recurrent osteomyelitis”, with its severe multifocal form “chronic recurrent multifocal osteomyelitis” (CRMO). For almost three decades, an infectious aetiology had been considered, since especially Propionibacterium acnes had been isolated from bone lesions of individual patients. However, this concept has been challenged since long-term antibiotic therapy did not alter the course of disease and modern microbiological techniques (including PCR) failed to confirm bone infection as an underlying cause. Over recent years, a profound dysregulation of cytokine expression profiles has been demonstrated in innate immune cells of CNO patients. A hallmark of monocytes from CNO patients is the failure to produce immune regulatory cytokines interleukin-10 (IL-10) and IL-19, which have been linked with genetic and epigenetic alterations. Subsequently, a significant upregulation of pro-inflammatory, NLRP3 inflammasome-dependent cytokines (IL-1β and TNF-α), has been demonstrated.
Summary
The current knowledge on CNO, the underlying molecular pathophysiology, and modern imaging strategies are summarized; differential diagnoses, treatment options, outcome measures, as well as quality of life studies are discussed.
IL-9-producing Th9 cells display a group of helper T cells with similarities to Th17 and Th2 T cells and have been shown to be involved in synovial inflammation in rheumatoid arthritis (RA) patients. So far, it is unclear which parameters drive Th9 differentiation in lymphocytes derived from RA patients compared to immunologically healthy individuals and whether autocrine mechanisms are able to enhance Th9 polarization. Further, parallel pathways of induction of IL-17-producing cells with Th9 phenotype have to be distinguished from exclusively Th9-inductive mechanisms. Thus, the present study aimed to determine the parameters of Th9 induction by simulation in a standardized inflammatory cytokine milieu.Peripheral naive and non-naive T cells of RA patients and healthy donors (HD) were cultured under Th9 and Th17-driving conditions and phenotypically analyzed by flow cytometry and molecular analysis.Our findings indicate a similar differentiation pathway of Th9 and Th17 cells and similar distributions of IL-9+ T cells in RA and HD regardless of Th9- or Th17-promoting cytokine milieus. Whereas the magnitude and direction of Th9- or Th17-polarization was about the same in RA and HD, IL-17+ CD4+ T cells were significantly stimulated by Th17-inducing conditions in HD. In conclusion, the results indicate that Th9- and Th17-inducing cytokine conditions mimicking autoimmune inflammation in RA may have similar stimulatory effects regarding polarization of peripheral naive and non-naive T cells into Th9 or Th17 cells. The results suggest that the differentiation of Th9 cells may be also induced by Th17-driving conditions.
Charakterisierung von IL-9+ T-Zellen bei Patientinnen und Patienten mit Rheumatoider Arthritis

Die Rheumatoide Arthritis ist eine chronische Autoimmunerkrankung, deren Pathogenese noch unvollständig verstanden ist. Eine Dysbalance von pro- und antiinflammatorischen T-Helferzellen ist an der Entzündungsreaktion wesentlich beteiligt. Die Rolle von proinflammatorischen Th17-Zellen und regulatorischen Tregs ist bereits recht gut erforscht und bietet bereits erfolgreiche Therapieansätze. Umstritten bleibt die Rolle der sogenannten Th9-Zellen, welche pro- und antiinflammatorische Wirkungen zu vereinen scheinen. Die vorliegende Untersuchung umfasst die Daten von 7 RA-Patientinnen und -Patienten und 10 gesunden Kontrollen. Nach Isolation von naiven CD4+ T-Zellen erfolgte eine durchflusszytometrische Phänotypisierung der T-Zellen. Diese Phänotypisierung zeigte keinen relevanten Unterschied zwischen Pat und HD. Im Anschluss an eine 3-tägige Zellkultur von naiven und nicht naiven CD4+ T-Zellen unter Th9- und Th17-Stimulation erfolgte eine Auswertung mittels Durchflusszytometrie, RT-PCR und ELISA. Hierbei wurde eine sehr ähnliche Wirkung beider Stimuli auf die T-Zelldifferenzierung, die Zytokinproduktion und die Expression von Oberflächenmarkern beobachtet. Insbesondere die IL-9-Produktion wurde durch beide Stimuli signifikant induziert. Es scheint, dass IL-9-produzierende Zellen durch unterschiedliche Differenzierungswege induziert werden. Dies unterstützt die kontroverse Hypothese, dass Th9-Zellen keinen eigenständigen T-Helferzellsubtyp darstellen. Im Vergleich zu den Pat in Remission war die induzierte Zytokinproduktion (IL-9, IL-17, IFNγ) bei den HD betont. Dies liegt vermutlich an unspezifischen Effekten durch die immunsuppressive Therapie. Bei den beiden Patientinnen und Patienten mit der höchsten Krankheitsaktivität jedoch wurde die höchste IL-9-Konzentration in den Überständen nach Stimulation festgestellt. Ein IL-9-Exzess bzw. eine IL-9-Blockade hatte keinen signifikanten Einfluss auf die T-Zelldifferenzierung, die Zytokinproduktion und die Expression der Oberflächenmarker und des Transkriptionsfaktors. Das Vorliegen eines relevanten IL-9-regulierenden, autokrinen Mechanismus ist daher unwahrscheinlich. Zusammenfassend ergeben sich Hinweise, dass es sich bei IL-9-produzierenden Zellen aufgrund der diversen Differenzierungswege nicht um einen eigenständige T-Helferzellsubtyp handelt. Es benötigt einen sehr gut ausbalancierten Zytokincocktail und vermutlich eine längere Kulturzeit, um tatsächlich Th9-Zellen zu polarisieren. Ob diese in vivo vorkommen und eine Rolle in der Pathogenese der RA spielen ist zum jetzigen Zeitpunkt ebenso unklar wie die Rolle des IL-9 in der Pathogenese der RA. Daher kommt IL-9 als therapeutisches Ziel nach jetziger Kenntnis nicht in Betracht.
Der klinische Stellenwert der Polysomnographie bei Kindern mit kraniofazialen Fehlbildungssyndromen

Schlaf- und nächtliche Atemstörungen sind ein häufiges Symptom bei Kindern mit kraniofazialen Fehlbildungen, da es durch die Mittelgesichtshypoplasie zu einer Einengung der Atemwege kommen kann. Wir haben bei 28 Kindern Polysomnographien vorgenommen, um differenziertere Aussagen über die Art der schlafbezogenen Apnoen, den Zusammenhang mit bestimmten Schlafstadien und Schlafarchitektur und Korrelationen mit der mental-kognitiven Entwicklung sowie der Lebensqualität machen zu können. Diese wurden mittels dreier Fragebögen und psychologischer Testung bestimmt. Bei 5 Patienten fand sich ein schweres, bei 9 ein mäßiges und bei 12 ein mildes obstruktives Schlaf-Apnoe-Syndrom, nur 2 Kinder hatten kein OSAS. Die Kinder mit Crouzon-Syndrom hatten überwiegend (9/14) ein mittel- bis schwergradiges OSAS, während die Kinder mit Apert-Syndrom mehrheitlich von einer leichten Atemstörung betroffen waren. Neben obstruktiven Ereignissen war auch die Zahl der zentralen Apnoen erhöht. Aufgrund der hohen Korrelationen zwischen OSAS-Schweregrad und Entsättigungsindex könnte die Pulsoxymetrie als ambulantes Screeningverfahren eingesetzt werden. Die Schlafarchitektur war nicht wesentlich beeinträchtigt bei einer eindrucksvollen Häufung der respiratorischen Ereignisse im REM-Schlaf (RDI 16,97/h in REM vs 8,2 in NREM) und einem in Abhängigkeit vom Schweregrad des OSAS erhöhten Arousalindex. Insbesondere nasaler CPAP und v. a. BiPAP, aber auch Adenotonsillektomien, führten zu einer deutlichen Besserung der respiratorischen Parameter. Aufgrund der Dynamik der Erkrankung sind engmaschige Kontrollen und ggf. Erweiterungen und Anpassungen der Therapie nötig. Die Lebensqualität lag insgesamt im Normbereich - unabhängig von der OSAS-Schwere - zeigte jedoch im Verlauf einen Zusammenhang mit dieser und der Klinik. Obwohl keine signifikanten Korrelationen zwischen Tagessymptomatik, einzelnen PSG-Parametern und OSAS-Schwere aufzuzeigen war, spiegelte sich in den Fragebögen eine erfolgreiche Therapie wider, so dass ein überarbeiteter Fragebogen als Therapie- und Verlaufskontrolle dienen könnte.
Diskussion der häufigsten soliden Hirntumorentitäten in der Pädiatrie. Analyse der im Rahmen der HIT-2000-Studie angefertigten Liquorpräparate sowie der im Rahmen dieser Studie gesammelten persönlichen Daten der betroffenen Kinder bezüglich der Häufigkeitsverteilung der einzelnen Entitäten, des Eintretens einer zytologischen / radiologischen Meningeose oder von soliden Metastasen, Vergeich der Sensitivität radiologischer und zytologischer Verfahren, Vergleich des jeweils möglichen Resektionsausmaßes, Häufigkeit des Auftretens von Rezidiven oder Todesfällen, Aussagemöglichkeiten der Zytologie.
In der vorliegenden Arbeit wurden am Beispiel der Diphtherie- und Tetanusimpfung sowohl die Langzeitwirkung von Impfungen als auch die akuten Effekte einer erneuten Auffrischimpfung auf die antigenspezifischen Memory-B-Zellen und die Antikörper-Titer untersucht. Dazu wurden antigenspezifische Antitoxin-Spiegel mittels ELISA und im Neutralisationstest sowie spezifische Memory-B-Zellen mit dem B-Zell-ELISPOT-Test bei 13 Probanden bestimmt. Es zeigte sich, dass sowohl der Antikörper-Titer als auch die Memory-B-Zellen über lange Zeit nach der Impfung persistieren, ohne jedoch miteinander zu korrelieren. Das spricht dafür, dass der Antikörper-Titer nicht, wie bisher angenommen, durch kontinuierliche Stimulierung und Differenzierung von Memory-B-Zellen zu kurzlebigen Plasmazellen aufrechterhalten wird. Die bislang nur in der Maus bewiesene Existenz langlebiger Plasmazellen wird damit auch für den Menschen wahrscheinlich. Weiterhin korrelierte die Anzahl bisheriger Auffrischimpfungen mit der Frequenz der spezifischen Memory-B-Zellen, nicht aber mit der Höhe des Antikörper-Titers. Damit konnte zum erstenmal gezeigt werden, dass die Langzeitimmunität nach Impfungen beim Menschen besser von den Memory-B-Zellen als von den Antikörper-Titern reflektiert wird. Anhand der Untersuchng der Immunreaktion nach eienr erneuten Diphtherie-Auffrischimpfung konnte gezeigt werden, dass die Boosterung der antigenspezifischen B-Zellen und des Antikörper-Titers einem gut kontrollierten Mechanismus der klonalen Expansion und Kontraktion unterliegt. Dabei bestand eine starke negative Korrelation zwischen der Anzahl der Boosterimmunisierungen und der Stärke der Expansion spezifischer Memory-B-Zellen bzw. Antikörper-Titer. Die Höhe der prävakzinalen spezifischen B-Zellfrequenzen und der Antikörper-Titer beeinflussten ebenfalls die Stärke der Immunantwort, so dass besonders solche Probanden von einer Aufrischimpfung profitierten, die vor der Impfung niedrige B-Zellfrequenzen bzw. Antikörper-Titer hatten. In dieser Arbeit konnte somit erstmals die Bedeutung von Memory-B-Zellen für die postvakzinale Langzeitimmunität beim Menschen aufgezeigt werden. Dieser Parameter sollte in Zukunft als zusätzliches Kriterium für die Beurteilung des Immunschutzes und des Zeitpunktes für Reimmunisierungen mit herangezogen werden.
