LASP-1, das LIM und SH3 Protein 1, ist ein Aktin-bindendes Gerüstprotein, das in verschiedenen Tumorentitäten überexprimiert ist. Dabei scheint LASP-1 eine wichtig Rolle sowohl bei der Proliferation und Migration von Zellen als auch bei der Tumorgenese und Metastasierung zu spielen.
Ziel dieser Arbeit war es, die Expression von LASP-1 im Urothelkarzinom der Harnblase zu untersuchen und eine daraus abzuleitende klinische Relevanz für die Diagnostik zu evaluieren. Dazu wurden histologische Blasenschnitte immunhistochemisch nach LASP-1 gefärbt und Western Blot-Analysen von Urinproben durchgeführt.
Die Auswertung der immunhistochemisch gefärbten Blasenschnitte ergab, dass Urothelkarzinome signifikant mehr LASP-1 auf Proteinebene exprimieren als gesundes Blasengewebe. Allerdings konnte keine Korrelation zwischen der Stärke der LASP-1-Expression und verschiedener klinisch-pathologischer Parameter nachgewiesen werden.
Mittels Western Blot-Analysen gelang es, LASP-1 eindeutig im Urin und statistisch signifikant häufiger bei Blasenkarzinompatienten zu detektieren. Ohne Berücksichtigung einer Kontamination mit LASP-1-positiven Blut- und Entzündungszellen ist der LASP-1-Nachweis im Western Blot mit einer Gesamtsensitivität von 84,2% derzeit sensitiver als die meisten erhältlichen Tumormarker. Darüber hinaus ergab der Vergleich von Spontanurin und von Harnblasenspülflüssigkeit, dass Spontanurinproben sogar geeigneter zur Diagnostik zu sein scheinen.
Abschließend kann zusammengefasst werden, dass LASP-1 aufgrund der einfachen, nicht invasiven und kostengünstigen Probengewinnung zusammen mit den hohen Werten für Sensitivität und Spezifität als Urin-basierter Tumormarker für das Urothelkarzinom der Harnblase vielversprechend zu sein scheint.
Die vorliegende Arbeit demonstriert an Hand von verschiedenen Modellsystemen wie detailliert sich die grundlegenden Eigenschaften molekularer Adsorbate mit der winkelaufgelösten Photoemission erkunden lassen. Die von Peter Puschnig et al. vorgestellte Verknüpfung zwischen Photoemissionsintensität und den Molekülorbitalen im Grundzustand mittels einer Fouriertransformation war dabei entscheidend, um die verschiedenen physikalischen Effekte einordnen und verstehen zu können. Während für Coronen oder HBC die Orbitale im Grundzustand sehr gut zum Experiment passen, lassen sich für PTCDA und NTCDA einige Abweichungen von der DFT-Rechnung auf Basis der (semi-)lokalen GGA- oder LDA-Funktionale erkennen, die sich bei Messungen mit s-Polarisation hervorheben lassen. Diese können auf den Einfluss des Endzustandes in der Photoemission zurückgeführt werden. Im Rahmen der Dysonorbitale lassen sich die dafür verantwortlichen Relaxationseffekte zwischen dem N-Elektronensystem des Moleküls im Grundzustand und dem (N-1)-Elektronensystem des zurückbleibenden Kations explizit beschreiben. Die Berechnung des Photoemissionssignals mittels Fouriertransformation des Grundzustandes kann darüber hinaus weitere physikalische Effekte nicht korrekt berücksichtigen. Erste Anzeichen hierfür konnten am PTCDA-HOMO bei einer Photonenenergie von 27 eV und s-Polarisation detektiert werden. Darüber hinaus kann die Näherung des Photoelektronenendzustands als ebene Welle den beobachteten zirkularen Dichroismus am HOMO und LUMO von PTCDA nicht erklären. Erst in der Erweiterung durch eine Partialwellenzerlegung des Photoelektronenendzustands tritt ein dichroisches Signal in der theoretischen Beschreibung auf. Für das delokalisierte pi-Elektronensystem von PTCDA ist aber selbst diese Verfeinerung noch nicht ausreichend, um das Experiment korrekt beschreiben und weitere Eigenschaften vorhersagen zu können. Qualitativ lassen sich die Veränderungen im CDAD bei der Transformation um 90° für HOMO und LUMO mit einem gruppentheoretischen Ansatz verstehen. Damit ist es möglich, den molekularen Zuständen ihre irreduzible Darstellung zuzuweisen, worüber sich für PTCDA die Verteilung der quantenmechanischen Phase rekonstruieren lässt. Dies ist deshalb äußerst bemerkenswert, da üblicherweise in physikalischen Experimenten nur die Intensität und keine Informationen über die Phase messbar sind. Damit können die Photoemissionsmessungen im k||-Raum vollständig in den Realraum transformiert werden, wodurch die laterale Ortsinformation über die höchsten besetzen Molekülorbitale von PTCDA zugänglich wird. Neben der Bestimmung der molekularen Orbitale, deren Struktur von der Anordnung der Atome im Molekül dominiert wird, enthält die winkelaufgelöste Photoemission Informationen über die Adsorbat-Substrat-Wechselwirkung. Für hoch geordnete Monolagen ist es möglich, die verschiedenen Verbreiterungsmechanismen zu trennen und zu analysieren. Bei den untersuchten Systemen sind die Verbreiterungen aufgrund von unterschiedlichen Adsorptionsplätzen oder Probeninhomogenitäten ebenso wie die experimentelle Auflösung der 2D-Analysatoren vernachlässigbar gegenüber Lebensdauereffekten und evtl. Verbreiterung aufgrund von Dispersionseffekten. Bereits bei den äußerst schwach wechselwirkenden Systemen Coronen auf Ag(111) und Au(111) unterscheiden sich die beiden Systeme in ihrer Lorentzverbreiterung beim HOMO. In erster Näherung lässt sich dies auf eine Lebensdauer des entstandenen Photolochs zurückführen, welches je nach Stärke der Substratkopplung unterschiedlich schnell mit Substratelektronen aufgefüllt werden kann. Die Lorentzbreite als Indikator für die Wechselwirkung bzw. Hybridisierungsstärke zeigt für die Systeme mit Ladungstransfer vom Substrat in das Molekül eine sehr viel größere Verbreiterung. Zum Beispiel beträgt die Lorentzbreite des LUMO für NTCDA/Ag(110) FWHM=427 meV, und somit eine mehr als fünfmal so große Verbreiterung als für das HOMO von Coronen/Au(111). Diese starke Verbreiterung geht im Fall von NTCDA/Ag(110) wie auch bei den untersuchten Systemen NTCDA/Cu(100) und PTCDA/Ag(110) einher mit einem Ladungstransfer vom Substrat ins Molekül, sowie mit der Ausbildung eines zusätzlichen charakteristischen Signals in der Winkelverteilung des LUMO, dem Hybridisierungszustand bei kx,y=0Å-1. Die Intensität dieses Zustands korreliert bei den Systemen NTCDA auf Cu(100) bzw. auf Ag(110) jeweils mit der Lorentzbreite des LUMO-Zustands. Die Hybridisierung zwischen Molekül und Substrat hat noch weitere Auswirkungen auf die beobachtbaren physikalischen Eigenschaften. So führt die starke Hybridisierung mit dem Substrat wiederum dazu, dass sich die intermolekulare Dispersion für die Elektronen im LUMO-Zustand deutlich verstärkt. Der direkte Überlapp der Wellenfunktionen ist im System PTCDA/Ag(110) laut DFT-Rechnungen relativ klein und führt lediglich zu einer Bandbreite von 60 meV. Durch die Hybridisierung mit den delokalisierten Substratbändern erhöht sich der Grad der Delokalisierung im LUMO-Zustand, d.h. die Bandbreite steigt auf 230 meV, wie das Experiment bestätigt. Im Gegensatz zu früheren STM/STS-basierten Messungen [Temirov2006] kann mit der Kombination aus DFT-Rechnung und ARPES-Experiment eindeutig nachgewiesen werden, dass das Substrat im Fall von PTCDA/Ag(110) die Bandbreite verstärken kann, sodass sich die effektive Masse der Lochladungsträger von meff=3,9me auf meff=1,1me reduziert. Im Blick auf die eingangs gestellte Frage, ob sich molekulare Adsorbate eher wie isolierte Moleküle oder als periodische Festkörper beschreiben lassen, kommt diese Arbeit auf ein differenziertes Ergebnis. In den Impulsverteilungen, die sich aus der Form der molekularen Wellenfunktionen ableiten lassen, spiegelt sich eindeutig der isolierte molekulare Charakter wieder. Dagegen zeigt sich in der Energiedispersion E(k||) ein delokalisierter, blochartiger Charakter, und es konnte demonstriert werden, dass es zu einem Vermischen von Metall- und Molekülwellenfunktionen kommt. Molekulare Adsorbate sind also beides, isolierte Moleküle und zweidimensionale Kristalle mit delokalisierten Zuständen.
Nuclear spins in motion is an intrinsic component of any dynamic process when studied using magnetic resonance imaging (MRI). Moving spins define many functional characteristics of the human body such as diffusion, perfusion and blood flow. Quantitative MRI of moving spins
can provide valuable information about the human physiology or of a technical system. In particular, phase-contrast MRI, which is based on two images with and without a flow-encoding gradient, has emerged as an important diagnostic tool in medicine to quantify human blood flow. Unfortunately, however, its clinical usage is hampered by long acquisition times which only provide mean data averaged across multiple cardiac cycles and therefore preclude Monitoring the immediate physiological responses to stress or exercise. These limitations are expected to be overcome by real-time imaging which constitutes a primary aim of this thesis.
Short image acquisition times, as the core for real-time phase-contrast MRI, can be mainly realized through undersampling of the acquired data. Therefore the development focused on related technical aspects such as pulse sequence design, k-space encoding schemes and image
reconstruction. A radial encoding scheme was experimentally found to be robust to motion and
less sensitive to undersampling than Cartesian encoding. Radial encoding was combined with a FLASH acquisition technique for building an efficient real-time phase-contrast MRI sequence.
The sequence was further optimized through overlapping of gradients to achieve the shortest possible echo time. Regularized nonlinear inverse reconstruction (NLINV), a technique which jointly estimates the image content and its corresponding coil sensitivities, was used for image
reconstruction. NLINV was adapted specifically for phase-contrast MRI to produce both Magnitude images and phase-contrast maps. Real-time phase-contrast MRI therefore combined two highly undersampled (up to a factor of 30) radial gradient-echo acquisitions with and without a
flow-encoding gradient with modified NLINV reconstructions. The developed method achieved
real-time phase-contrast MRI at both high spatial (1.3 mm) and temporal resolution (40 ms).
Applications to healthy human subjects as well as preliminary studies of patients demonstrated
real-time phase-contrast MRI to offer improved patient compliance (e.g., free breathing) and immediate access to physiological variations of flow parameters (e.g., response to enhanced intrathoracic pressure). In most cases, quantitative blood flow was measured in the ascending aorta as an important blood vessel of the cardiovascular circulation system commonly studied
in the clinic. The performance of real-time phase-contrast MRI was validated in comparison to standard Cine phase-contrast MRI using studies of flow phantoms as well as under in vivo conditions. The evaluations confirmed good agreement for comparable results.
As a further extension to real-time phase-contrast MRI, this thesis implemented and explored a dual-echo phase-contrast MRI method which employs two sequential gradient echoes with and without flow encoding. The introduction of a flow-encoding gradient in between the two echoes
aids in the further reduction of acquisition time. Although this technique was efficient under in vitro conditions, in vivo studies showed the influence of additional motion-induced Phase contributions. Due to these additional temporal phase information, the approach showed Little promise for quantitative flow MRI.
As a further method three-dimensional real-time phase-contrast MRI was developed in this thesis to visualize and quantify multi-directional flow at about twice the measuring time of the standard real-time MRI method, i.e. at about 100 ms temporal resolution. This was achieved
through velocity mapping along all three physical gradient directions. Although the method is still too slow to adequately cover cardiovascular blood flow, the preliminary results were found to be promising for future applications in tissues and organ systems outside the heart. Finally, future developments are expected to benefit from the adaptation of model-based reconstruction
techniques to real-time phase-contrast MRI.
Da Defekte im Bereich der oralen Schleimhaut infolge von Traumata, angeborenen sowie erworbenen Krankheiten die ungestörte Funktionsweise in Bezug auf Atmung, Nahrungsaufnahme und Sprache des Menschen empfindlich beeinträchtigen und ein adäquater, alle Funktionen wiederherstellender Wundverschluss mit dem limitierten Eigengewebe oft nicht möglich ist, bietet das Tissue Engineering durch die Entwicklung eines Hautäquivalents eine aussichtsreiche Alternative. Um eine ausreichende Menge an Zellen für die Herstellung eines autologen Transplantates in kurzer Zeit zur Verfügung zu stellen, sollte in der vorliegenden Arbeit eine Methode zur Steigerung der Proliferationsfähigkeit primärer humaner Keratinozyten aus oraler Mukosa im Zellkultursystem etabliert werden. Dazu mussten zunächst über die Explantation von Gewebeproben gesunder Patienten orale Schleimhautzellen gewonnen und die primären Keratinozyten von den mitwachsenden Fibroblasten isoliert werden. Dies wurde durch chemische und mechanische Separationsmethoden erreicht. Die Kultivierung der exprimierten Zellen erfolgte unter ständiger Beobachtung und physiologischen Bedingungen über einen Zeitraum von mehreren Wochen. Nach Konfluenz der zweiten Passage wurden die Zellen geerntet und für die Versuche vorbereitet. Die Steigerung der Proliferationsfähigkeit der Keratinozyten sollte durch die Anreicherung epidermaler Stammzellen erreicht werden, da diese insbesondere durch ihre Fähigkeit zur asymmetrischen Teilung die Grundlage für die Regeneration, Differenzierung und Homöostase des Gewebes bilden. Eine Möglichkeit zur Isolation von Zellen mit Stammzelleigenschaften stellt die Adhäsion an beschichteten Zellkulturgefäßen dar. Die Affinität des hauptsächlich in Stammzellen vorkommenden ß1‐Integrin‐Rezeptors zu Bestandteilen der Basalmembran wie Kollagen‐IV und Laminin sollte die Trennung hoch proliferativer Zellen von weniger teilungsaktiven Zellen leisten und das Protein indirekt als Marker für die Stammzellen fungieren. Über die Adhäsion der Keratinozyten an mit den Komponenten Kollagen‐IV und Laminin beschichteten Gefäßen ließen sich zwei Zellpopulationen (adhärente und nicht-adhärente Zellen) gewinnen. Unabhängig von der verwendeten Adhäsionskomponente zeigten die Fraktionen den charakteristischen Wachstumsverlauf (lag‐Phase, log‐ Phase, stationäre Phase und Absterbephase) in vitro kultivierter Zellen, allerdings konnte kein signifikanter Unterschied in Bezug auf die Vitalität und die Proliferationskinetik der Keratinozyten festgestellt werden. Eine nach der geleisteten Auftrennung der Keratinozyten zwischengeschaltete Analyse und Identifikation von Stammzellen mittels ß1‐Integrin‐Marker (z.B. durch einen Immunfluoreszenztest) könnte klären ob die adhärente Population überhaupt einen erhöhten Anteil an hoch proliferativen Keratinozyten beinhaltet oder ob die zahlreichen notwendigen, aber für die Zellen belastenden, Zwischenschritte der hier angewendeten indirekten Methode auslösend für die geringen Unterschiede sind. In Anlehnung an die von Stein et al. erarbeiteten guten Ergebnisse bezüglich der Proliferationskapazität oraler Keratinozyten nach Adhäsion an Kollagen‐IV‐beschichteten Zellkulturgefäßen wurde bei der vorliegenden Arbeit auf die aufwändige immunhistochemische Untersuchung verzichtet. Ein verstärktes Wachstum der adhärenten Population konnte nur bei vereinzelten Proben festgestellt werden; insgesamt konnte die prioritär gewünschte Steigerung der Proliferation primärer humaner Keratinozyten im Zellkultursystem zur raschen Bereitstellung von Zellen für die Entwicklung eines autologen Mundschleimhaut‐Transplantates nicht erreicht werden. Die drei angewandten Verfahren zur Erfassung der Quantität führten hinsichtlich der Wachstumssteigerung zu ähnlichen Ergebnissen. Da sie aber zum einen durch das Wegfallen der für die Zellzählung und den WST‐1‐Test notwendigen Zwischenschritte eine non‐invasive (ohne mechanische Irritation und Interaktion mit Zusatzstoffen), d.h. für die Zellen schonende Methode darstellt und sich zum anderen die Ergebnisse der Real-Time-Zellanalyse, im Gegensatz zur Endpunkt-Messung, direkt auf die vorangegangenen Messungen beziehen, überzeugte die Auswertung mittels Impedanzmessung in Genauigkeit und Darstellung der Veränderung des Zellwachstums über die Zeit.
Retrospektive Studie über die Therapie von Kieferzysten an der Klinik und Poliklinik für Mund-, Kiefer- und Plastische Gesichtschirurgie der Universität Würzburg. 285 Patienten wurden aufgrund einer Kieferzyste, von 1997-2009, stationär behandelt. 191 odontogene Zysten, 77 nichtodontgene Zysten, elf nichtepitheliale und drei dysgenetische Zysten wurden operativ entfernt. Drei Patienten wurden sowohl an einer odontogenen bzw. nichtodontogenen Zyste operiert.Vorweigend erfolgte die Operation durch Zystektomie.
Seit März 2004 ist im Zentrum für operative Medizin der Universitätsklinik Würzburg der Schockraum mit einem Somatom Sensation 16 der Firma Siemens, einem CT mit Sliding-Gantry in Betrieb. Auch schwer verletzte Kinder werden hier nach dem Würzburger Schockraumalgorithmus behandelt. Insgesamt gibt es nur wenige Daten zur Anwendung von GK-CT bei Kindern. Die DGU beschreibt im TraumaRegister die Daten von zuletzt 266 Kliniken. Nur 5,1 % dieser Daten stammen von Kindern unter 16 Jahren. Die Unterschiede zu Erwachsenen entstehen durch die geringere Erfahrung im Straßenverkehr, die kindliche Anatomie, die größere Gefährdung durch den hypovolämischen Schock und die bessere Rekonvaleszenz von Kindern. Ebenso ist die radiologische Diagnostik durch das geringere Körpervolumen und die höhere Strahlenempfindlichkeit der Kinder problematischer. Der Schockraumalgorithmus der Universitätsklinik Würzburg besteht aus der Reanimations-, CT- und Versorgungsphase. Die Indikation zur GK-CT wird nach dem Triage-Schema gestellt und ist auch bei polytraumatisierten Kindern Diagnostik der Wahl. Die Schwere der Verletzungen wird mit dem ISS bestimmt. In den Jahren 2004 bis 2009 wurden 72 Kinder mit Verdacht auf ein Polytrauma behandelt. Davon erhielten 57 (79,2%) ein GK-CT, 15 (20,1 %) erhielten eine organbezogene CT (CCT, Thorax, Abdomen). 15 (26,3 %) der 57 Kinder mit GK-CT hatten retrospektiv kein Polytrauma (ISS < 16), wobei hier von einer Overtriage auszugehen ist. Die Kinder wurden hauptsächlich im Straßenverkehr und hier als Beifahrer eines PKWs verletzt, gefolgt von den Radfahrern und Fußgängern, die von einem PKW erfasst wurden. Ein Drittel der Patienten erlitt Sturzverletzungen aus bis zu fünf Metern Höhe. Die am häufigsten verletzte Körperregion war mit 52,8 % der Kopf, gefolgt vom Thorax, den Extremitäten und 25 % Gesichtsverletzungen. Dies ist durch die ungünstigere Körper-Kopf-Relation der Kinder erklärbar. Im Vergleich zu Erwachsenen ist bei Kindern vermehrt das Gesicht, der Hals und die untere Extremität verletzt, die Wirbelsäule war weniger betroffen. 82 % der Kinder wurden maschinell beatmet, davon 65% initial und 14% in der ZNA. Es wird empfohlen, Kinder frühzeitig zu intubieren, was sich in der Intubationsrate von 82% widerspiegelt. Eine Notfalloperation wurde in 38,9 % der Fälle durchgeführt, wobei es sich hauptsächlich um Frakturrepositionen handelte, vor allem bei Kindern mit ISS < 16. Bei Kindern mit ISS ≥ 16 wurde in 8,8 % der Fälle eine Kraniotomie durchgeführt. Insgesamt wurden 62 % der Kinder operiert, wobei es sich auch bei den Operationen im Verlauf hauptsächlich um Frakturrepositionen handelte. 15 Kinder erhielten kein GK-CT als primäre Diagnostik, obwohl dies retrospektiv gemäß des Triage-Schemas indiziert gewesen wäre. Bei diesen Kindern wurden zusätzlich zum CCT selektive CTs, Röntgenaufnahmen und Abdomenultraschalluntersuchungen durchgeführt. Bei sechs Kindern wurde trotz initialer GK-CT eine therapierelevante Diagnose erst im Verlauf des Klinikaufenthalts gestellt. Dabei handelte es sich um eine Schädelbasis-, eine Orbitaboden-, eine Klavikula-, eine Talusfraktur, ein Mesenterialarterieneinriss und eine Ileumruptur. Ein Kind verstarb, weil dessen Ileumruptur in der radiologischen Diagnostik nicht detektiert wurde und die Zeit bis zur Laparotomie verzögerte. Während des Krankenhausaufenthalts wurde zur Verlaufsdokumentation die Röntgenuntersuchung, gefolgt von der Sonographie, am häufigsten angewendet. Die CT tritt hierbei in den Hintergrund, wobei die MRT häufiger durchgeführt wurde als bei der initialen Diagnostik.
Die Strahlenbelastung des pädiatrischen Kollektivs ist im Kontext der schnellen Diagnoseerhebung therapierelevanter Verletzungen zu sehen. Bei der GK-CT ist mit einem durchschnittlichen DLP von 2268 mGy•cm, bei der CCT von 956 mGy•cm zu rechnen. Bei den Röntgenuntersuchungen ein DFP von 4 cGy•cm2 bei Thoraxröntgenuntersuchungen und 11 cGy•cm2 bei anderen Röntgenuntersuchungen. Zur korrekten Einordnung der Strahlendosis sind genauere Richtwerte für Kinder und genauere Protokolle nötig. Die GK-CT ist Methode der Wahl bei polytraumatisierten Kindern und wird nur in seltenen Fällen von der FAST oder der Röntgenuntersuchung ersetzt. Trotz der GK-CT können nicht alle therapierelevanten Diagnosen gestellt werden. Insgesamt verbrachten die Kinder durchschnittlich 17 Tage im Krankenhaus und sieben Tage auf der Intensivstation. 77,8 % konnten danach nach Hause entlassen werden, sieben Kinder mussten nach dem Krankenhausaufenthalt in eine Rehabilitationsklinik verlegt werden. Die Letalität der Kinder ist mit 10,5% (n=6) im Vergleich zu den Erwachsenen mit 13,5 % und anderen Studien mit einer pädiatrischen Letalität mit durchschnittlich 18 % geringer. Vier Kinder verstarben an schwerem SHT, ein Kind in Folge seines schweren Thoraxtraumas und ein Kind entwickelte eine Sepsis in Folge einer in der radiologischen Diagnostik nicht detektierbaren Ileumruptur.
Die radiologische Diagnostik des kindlichen Polytraumas hat sich durch das MSCT in den letzten Jahren stark gewandelt. Die geringe Letalität unseres Patientenkollektivs ist ein deutlicher Hinweis auf die Effizienz des Würzburger Schockraumalgorithmus auch beim pädiatrischen Polytrauma.
Testing Models with Higher Dimensional Effective Interactions at the LHC and Dark Matter Experiments

Dark matter and non-zero neutrino masses are possible hints for new physics beyond the Standard Model of particle physics. Such potential consequences of new physics can be described by effective field theories in a model independent way. It is possible that the dominant contribution to low-energy effects of new physics is generated by operators of dimension d>5, e.g., due to an additional symmetry. Since these are more suppressed than the usually discussed lower dimensional operators, they can lead to extremly weak interactions even if new physics appears at comparatively low scales. Thus neutrino mass models can be connected to TeV scale physics, for instance. The possible existence of TeV scale particles is interesting, since they can be potentially observed at collider experiments, such as the Large Hadron Collider. Hence, we first recapitulate the generation of neutrino masses by higher dimensional effective operators in a supersymmetric framework. In addition, we discuss processes that can be used to test these models at the Large Hadron Collider. The introduction of new particles can affect the running of gauge couplings. Hence, we study the compatibilty of these models with Grand Unified Theories. The required extension of these models can imply the existence of new heavy quarks, which requires the consideration of cosmological constraints. Finally, higher dimensional effective operators can not only generate small neutrino masses. They also can be used to discuss the interactions relevant for dark matter detection experiments. Thus we apply the methods established for the study of neutrino mass models to the systematic discussion of higher dimensional effective operators generating dark matter interactions.
Das Nebennierenrindenkarzinom ist eine hochmaligne Erkrankung und hat eine schlechte Prognose. Mitotane ist bis heute die einzige hierfür zugelassene Therapie. Um die molekularen Mechanismen der Mitotanetherapie besser zu verstehen, wurde die Nebennierenkarzinom-Zelllinie NCI-H295 mit unterschiedlichen Konzentrationen von Mitotane inkubiert und die Wirkung auf mehreren Ebenen untersucht. Dabei kam der Untersuchung der Steroidogenese und apoptotischer Vorgänge ein besonderer Fokus zu. In den Hormonanalysen via Immunoassay zeigte sich eine zeit- und konzentrationsabhängige Hemmung der adrenalen Steroidsekretion. So kam es unter 24-stündiger Inkubation mit 100µM Mitotane zu einer Reduktion der Cortisolsekretion um 89%. Diese Hormonsuppression geht einher mit einer Herabregulation von steroidogenen Enzymen in den durchgeführten Microarray-basierten Genexpressionsanalysen. So konnte gezeigt werden, dass vor allem Steroidbiosynthese-Enzyme der Zona fasciculata und reticularis betroffen sind. Als weitere wichtige Gene im Zusammenhang mit der Beeinflussung des Steroidhaushalts unter Mitotanetherapie konnten SQLE, LDLR, SCD, SREBF1 und ABCG1 identifiziert werden.
Gleichzeitig konnte durch Durchflusszytometrie und Zelltod-ELISA die proapoptotische Wirkung von Mitotane gezeigt werden (FACS: 100µM Mitotane, 24 Stunden; Zunahme der Apoptose um den Faktor 2,13). Dies bestätigte sich beispielsweise auch in der Überexpression des Apoptosegens BAX in der Real-Time-PCR. Weiterhin zeigte der RNA-Microarray eine starke Expressionszunahme bei Genen, die mit dem programmierten Zelltod zusammenhängen wie GDF15, DUSP4, TRIB3 und CHOP.
Ausgehend von den klinischen Effekten und bestätigt durch die oben genannten in vitro Ergebnisse bewirkt Mitotane auch molekular folgende Änderungen in Nebennierenrindenzellen: Hemmung der Steroidogenese und Induktion von Apotose. Es stellt sich damit die Frage, ob diese Mechanismen parallel und separat voneinander ablaufen oder ob es einen gemeinsamen Nenner gibt.
Interessanterweise ergab die Analyse der Genexpressionsdaten, dass viele der proapoptotischen Gene mit dem sogenannten ER-Stress zusammenhängen. Einerseits könnte Mitotane durch direkte Inhibition der Hormonsekretion wirken, andererseits könnte ER-Stress durch Mitotane-induzierte-Bildung toxischer Lipide, wie Cholesterol, ausgelöst werden. Um den genauen Wirkmechanismus endgültig zu klären, werden weitere Experimente benötigt.
Mitotane-induzierter ER-Stress liefert einen vollständig neuen Blickwinkel auf die molekulare Wirkweise von Mitotane auf Nebennierenrindenkarzinomzellen. Gerade da die Mediatoren des ER-Stresses gut definiert und ER-Stress spezifisch sind, könnten sie sinnvolle Ziele in der Therapie darstellen. Die Beobachtung, dass Mitotane ER-Stress hervorruft, könnte in Zukunft somit zur Entwicklung wirksamerer und spezifischerer Therapien des Nebennierenrindenkarzinoms führen und so die infauste Prognose dieser malignen Krankheit verbessern.
Ziele. Die Zielsetzung der vorliegenden Arbeit war eine Bewertung der Versorgungslage von Personen mit glücksspielbezogenen Problemen in Deutschland. Dabei wurden 1) der Zugang zum Versorgungssystem, nämlich 1.1) Problembewusstsein bzgl. glücksspielbezogener Probleme und Erreichbarkeit von Glücksspielern über das Internet und 1.2) Faktoren der Inanspruchnahme von Hilfsangeboten untersucht sowie 1.3) eine Charakterisierung der Klientel in ambulanten Suchthilfeeinrichtungen und deren Zugang zum Hilfesystem vorgenommen. Zudem wurden in ambulanten Suchthilfeeinrichtungen 2) die erbrachten Leistungen für Personen mit glücksspielbezogenen Problemen und Einflussfaktoren auf die Versorgungsnutzung bzw. den Behandlungsverlauf sowie 3) das Behandlungsergebnis einer Analyse unterzogen.
Methodik. Die Arbeit basiert auf drei Studien: einer Onlinestudie (OS), einer Versorgungsstudie (VS) und einer Bevölkerungsstudie (BS). In der OS wurde eine Gelegenheitsstichprobe von Personen, die einen im Internet bereitgestellten Selbsttest zu pathologischem Glücksspielen (PG) vollständig ausfüllten (n=277) und bei Erfüllen der Einschlusskriterien anschließend an einer vertiefenden Studie teilnahmen (n=52), hinsichtlich soziodemographischer Charakteristika, Spielverhalten und spielbezogener Motive, PG inklusive Folgen, Beratungserfahrungen sowie psychopathologischen Variablen untersucht. In den anderen Studien wurden weitestgehend dieselben Instrumente verwendet. Die VS war eine Verlaufsstudie (Messzeitpunkte: Behandlungsbeginn und -ende), bei der konsekutiv Klienten aus n=36 ambulanten Suchthilfeeinrichtungen in Bayern aufgenommen wurden, die sich zwischen April 2009 und August 2010 vorstellten (n=461). Für die BS wurden Daten aus dem Epidemiologi-schen Suchtsurvey 2006 und 2009 herangezogen, einer Deutschland weiten repräsentativen Querschnittbefragung 18- bis 64-Jähriger Erwachsener (2006: n=7.810; 2009: n=8.002).
Ergebnisse. Zum Zugang zur Versorgung sind 1.1) über das Internet erreichbare Glücksspieler hauptsächlich junge, ledige Männer, von denen ungefähr die Hälfte die Diagnose PG erfüllen. Anhand der Spielmotive lassen sich drei Spielerklassen abbilden, die sich hinsichtlich ihres Schweregrads von PG unterschieden. Die Bereitschaft zur Teilnahme an einem Online-Präventionsprogramm hängt hauptsächlich mit der Anzahl erfüllter DSM-IV-Kriterien für PG zusammen. Im Gegensatz zur VS sind die online erreichten Glücksspieler jünger und zu einem höheren Anteil subklinisch pathologische Glücksspieler (SPG, ein bis vier erfüllte DSM-IV-Kriterien für PG). 1.2) Hinsichtlich der Faktoren der Inanspruchnahme bestätigen sich systematische Unterschiede zwischen Glücksspielern in Behandlung und nicht in Behandlung. Ebenso zeigen sich Unterschiede zwischen SPGr und pathologischen Glücksspielern (PGr). Dabei ist die Anzahl erfüllter DSM-IV-Kriterien für PG der wichtigste Prädiktor für einen positiven Behandlungsstatus. Auch soziodemographische Merkmale, insbesondere Alter und Staatsangehörigkeit, spielen eine Rolle. 1.3) Die Mehrheit der Klienten in ambulanten Suchthilfeeinrichtungen ist männlich, durchschnittlich 37 Jahre alt, ledig und kinderlos und hat häufig eine ausländische Staatsbürgerschaft. Die am häufigsten gespielte und bevorzugte Spielform ist das Spielen an Geldspielautomaten. Viele der Klienten haben bereits im Vorfeld Hilfe in Anspruch genommen und Gründe für die Vorstellung in der Beratungsstelle waren in ungefähr drei Viertel der Fällen finanzielle Probleme und bei ungefähr der Hälfte Probleme in der Partnerschaft. In der ambulanten Suchthilfe sind 2) Prädiktoren für eine längere Kontaktdauer u.a. der Einbezug der Familie und Gruppengespräche. Behandlungsabbrüche werden u.a. durch eine nicht-deutsche Staatsbürgerschaft und höhere Spielfrequenz vorhergesagt. 3) Reguläre Beendigung und höhere Kontaktzahl sind u.a. Prädiktoren für eine Verbesserung der Glücksspielsymptomatik.
Schlussfolgerungen. Vor dem Hintergrund der Ergebnisse werden Implikationen für die Weiterentwicklung des Versorgungssystems zum einen im Sinne einer Erweiterung und Anpassung der Versorgungsstrukturen abgeleitet, wobei auf Information und Aufklärung, Früherkennung und Frühintervention mit einem Fokus auf Online-Angeboten sowie zielgruppen-spezifische Angebote unter anderem für Angehörige eingegangen wird. Auch die Wichtigkeit der Vernetzung verschiedener an der Beratung/Behandlung von PGr beteiligten Einrichtungen wird herausgestellt. Zum anderen beziehen sich die diskutierten möglichen Weiterentwicklungen auf das Versorgungsangebot und Behandlungsmerkmale, was Therapieumfeld/-voraussetzungen, Therapieplanung sowie therapeutische Maßnahmen beinhaltet.
In food and pharmaceutical analysis, the classical indices peroxide value (PV), acid value (AV) and p-anisidine value (ANV) still play an important role as quality and authenticity control parameters of fats and oils. These indices are sum parameters for certain deterioration products (PV for hydroperoxides, AV for free fatty acids, ANV for aldehydes) and are obtained using volumetric or UV/VIS spectroscopic analytical approaches. 1H NMR spectroscopy provides a fast and simple alternative to these classical approaches. In the present work, novel 1H NMR methods to determine hydroperoxides, free fatty acids and aldehydes in fats and oils were developed.
Hydroperoxides:
The influence of solvent, water, free fatty acids and sample weight on the hydroperoxide group proton (OOH) signal was investigated. On the basis of the obtained results, the sample preparation procedure of the new 1H NMR method was established. A rough assignment of the hydroperoxide group signals in edible fats and oils to methyl oleate, methyl linoleate and methyl linolenate was conducted. Furthermore, to gain information on how many different hydroperoxide species originate from trioleate autoxidation, a kinetic study on trioleate monohydroperoxides was performed. The evaluation of the data strongly indicates that all of the conceivable 18 trioleate monohydroperoxides were formed during trioleate autoxidation. The analytical performance of the NMR method was compared to that of the classical PV approach by means of the so-called “relative sensitivity” according to Mandel. It was shown that both methods exhibit a similar analytical performance. A total of 444 edible oil samples were analysed using both methods. For some oil varieties considerable discrepancies were found between the results. In the case of black seed oil and olive oil two substances were identified that influence the classical PV determination and thus cause positive (black seed oil) and negative (olive oil) deviations from the theoretical PV expected from the NMR values.
Free fatty acids:
In order to find the optimal solvent mixture to measure the carboxyl group protons (COOH) of free fatty acids in fats and oils, the effect of solvent on the COOH signal was investigated for different mixtures of CDCl3 and DMSO-d6. The comparison of the NMR method with the classical AV method by means of the relative sensitivity revealed that both methods exhibit a similar analytical performance. 420 edible oil samples were analysed by both approaches. Except for pumpkin seed oil, where slight deviations were observed, there was a good compliance between the results obtained from the two methods. Furthermore, the applicability of the 1H NMR assay to further lipids with relevance in pharmacy was tested. For hard fat, castor oil, waxes and oleyl oleate modifications of the original sample preparation procedure of the NMR method were necessary to achieve comparable results for both methods.
Aldehydes:
The new 1H NMR method enables the determination of the molar amounts of n-alkanals, (E)-2-alkenals and (E,E)-2,4-alkadienals. It was illustrated that the ANV can be modelled as a linear combination of the NMR integrals of these aldehyde species. A functional relationship was derived on the basis In conclusion, the new 1H NMR methods provide an excellent alternative to of calibration experiments. The suitability of the model was shown by comparing the NMR-determined ANVs with the measured classical ANVs of 79 commercially available edible oils of different oil types.
In conclusion, the new 1H NMR methods provide an excellent alternative to the determination of the classical indices PV, AV and ANV. They have several advantages over the classical methods including the consumption of small solvent amounts, the ability to automatize measurement and to acquire several different parameters out of the same NMR spectrum. Especially concerning their selectivity, the 1H NMR methods are highly superior to the classical methods.
Inflammation and oxidative stress are known to be involved in the pathogenesis of chronic kidney disease in humans, and in chronic renal failure (CRF) in rats. The aim of this work was to study the role of inflammation and oxidative stress in adenine-induced CRF and the effect thereon of the purported nephroprotective agent gum arabic (GA). Rats were divided into four groups and treated for 4 weeks as follows: control, adenine in feed (0.75%, w/w), GA in drinking water (15%, w/v) and adenine+GA, as before. Urine, blood and kidneys were collected from the rats at the end of the treatment for analysis of conventional renal function tests (plasma creatinine and urea concentration). In addition, the concentrations of the pro-inflammatory cytokine TNF-a and the oxidative stress markers glutathione and superoxide dismutase, renal apoptosis, superoxide formation and DNA double strand break frequency, detected by immunohistochemistry for
c-H2AX, were measured. Adenine significantly increased the concentrations of urea and creatinine in plasma, significantly decreased the creatinine clearance and induced significant increases in the concentration of the measured inflammatory mediators.
Further, it caused oxidative stress and DNA damage. Treatment with GA significantly ameliorated these actions. The mechanism of the reported salutary effect of GA in adenine-induced CRF is associated with mitigation of the adenine-induced inflammation and generation of free radicals.
Background
Chronic osteomyelitis due to direct bone trauma or vascular insufficiency is a frequent problem in orthopaedic surgery. In contrast, acute haematogenous osteomyelitis represents a rare entity that almost exclusively affects prepubescent children or immunodeficient adults.
Case Presentation
In this article, we report the case of acute pneumococcal osteomyelitis of the humerus in an immunocompetent and otherwise healthy 44-year-old male patient presenting with minor inflammation signs and misleading clinical features.
Conclusions
The diagnosis had to be confirmed by open biopsy which allowed the initiation of a targeted therapy. A case of pneumococcal osteomyelitis of a long bone, lacking predisposing factors or trauma, is unique in adults and has not been reported previously.
Introduction
Sudden tetraparesis represents a neurological emergency and is most often caused by traumatic spinal cord injury, spinal epidural bleeding or brainstem ischemia and less frequently by medial disc herniation or spinal ischemia.
Case presentation
Here we report the rare case of an 82-year-old Caucasian man who developed severe tetraparesis four days after radical cystoprostatectomy. An emergency diagnostic study for spinal cord affection was normal. Brain magnetic resonance imaging revealed acute bilateral ischemic strokes in the precentral gyri as the underlying cause.
Conclusions
This case report underlines the need to also consider unusual causes of tetraparesis in an emergency situation apart from spinal cord or brain stem injury in order not to leave severe symptomatology unclear and possibly miss therapeutic options.
We calculate two-dimensional (2D) spectra reflecting the time-dependent electronic predissociation of a diatomic molecule. The laser-excited electronic state is coupled non-adiabatically to a fragment channel, leading to the decay of the prepared quasi-bound states. This decay can be monitored by the three-pulse configuration employed in optical 2D spectroscopy. It is shown that in this way it is possible to state-selectively characterize the time-dependent population of resonance states with different lifetimes. A model of the NaI molecule serves as a numerical example.
Heparins are one of the most used class of anticoagulants in daily clinical practice. Despite their widespread application immune-mediated hypersensitivity reactions to heparins are rare. Among these, the delayed-type reactions to s.c. injected heparins are well-known usually presenting as circumscribed eczematous plaques at the injection sites. In contrast, potentially life-threatening systemic immediate-type anaphylactic reactions to heparins are extremely rare. Recently, some cases of non-allergic anaphylaxis could be attributed to undesirable heparin contaminants.
A 43-year-old patient developed severe anaphylaxis symptoms within 5–10 minutes after s.c. injection of enoxaparin. Titrated skin prick testing with wheal and flare responses up to an enoxaparin dilution of 1:10.000 indicated a probable allergic mechanism of the enoxaparin-induced anaphylaxis. The basophil activation test as an additional in-vitro test method was negative. Furthermore, skin prick testing showed rather broad cross-reactivity among different heparin preparations tested.
In the presented case, history, symptoms, and results of skin testing strongly suggested an IgE-mediated allergic hypersensitivity against different heparins. Therefore, as safe alternative anticoagulants the patient could receive beneath coumarins the hirudins or direct thrombin inhibitors. Because these compounds have a completely different molecular structure compared with the heparin-polysaccharides.
The Factorization Method is a noniterative method to detect the shape and position of conductivity anomalies inside an object. The method was introduced by Kirsch for inverse scattering problems and extended to electrical impedance tomography (EIT) by Brühl and Hanke. Since these pioneering works, substantial progress has been made on the theoretical foundations of the method. The necessary assumptions have been weakened, and the proofs have been considerably simplified. In this work, we aim to summarize this progress and present a state-of-the-art formulation of the Factorization Method for EIT with continuous data. In particular, we formulate the method for general piecewise analytic conductivities and give short and self-contained proofs.
Background
Malignant hyperthermia (MH), a metabolic myopathy triggered by volatile anesthetics and depolarizing muscle relaxants, is a potentially lethal complication of general anesthesia in susceptible patients. The implementation of modern inhalation anesthetics that research indicates as less potent trigger substances and the recommended limitations of succinylcholine use, suggests there may be considerable decline of fulminant MH cases. In the presented study, the authors analyzed suspected MH episodes during general anesthesia of patients that were referred to the Wuerzburg MH unit between 2007 and 2011, assuming that MH is still a relevant anesthetic problem in our days.
Methods
With approval of the local ethics committee data of patients that underwent muscle biopsy and in vitro contracture test (IVCT) between 2007 and 2011 were analyzed. Only patients with a history of suspected MH crisis were included in the study. The incidents were evaluated retrospectively using anesthetic documentation and medical records.
Results
Between 2007 and 2011 a total of 124 patients were tested. 19 of them were referred because of suspected MH events; 7 patients were diagnosed MH-susceptible, 4 MH-equivocal and 8 MH-non-susceptible by IVCT. In a majority of cases masseter spasm after succinylcholine had been the primary symptom. Cardiac arrhythmias and hypercapnia frequently occurred early in the course of events. Interestingly, dantrolene treatment was initiated in a few cases only.
Conclusions
MH is still an important anesthetic complication. Every anesthetist must be aware of this life-threatening syndrome at any time. The rapid onset of adequate therapy is crucial to avoid major harm and possibly lethal outcome. Dantrolene must be readily available wherever MH triggering agents are used for anesthesia.
Background
Positive associations have been found between quality of life, emotion regulation strategies, and heart rate variability (HRV) in people without intellectual disabilities. However, emotion regulation and HRV have rarely been investigated in people with intellectual disabilities. Assessment of subjectively reported quality of life and emotion regulation strategies in this population is even more difficult when participants are also visually impaired.
Methods
Subjective and objective quality of life, emotion regulation strategies, and HRV at rest were measured in a sample of people with intellectual disabilities and concomitant impaired vision (N = 35). Heart rate was recorded during a 10 min resting period. For the assessment of quality of life and emotion regulation, custom made tactile versions of questionnaire-based instruments were used that enabled participants to grasp response categories.
Results
The combined use of reappraisal and suppression as emotion regulation strategies was associated with higher HRV and quality of life. HRV was associated with objective quality of life only. Emotion regulation strategies partially mediated the relationship between HRV and quality of life.
Conclusions
Results replicate findings about associations between quality of life, emotion regulation, and HRV and extend them to individuals with intellectual disabilities. Furthermore, this study demonstrated that quality of life and emotion regulation could be assessed in such populations even with concomitant impaired vision with modified tactile versions of established questionnaires. HRV may be used as a physiological index to evaluate physical and affective conditions in this population.
Background
Metastatic melanoma of the seminal vesicles is a very rare clinical entity and has been reported only once until today in a patient suffering from concomitant HIV infection 12 years ago.
Case presentation
We report a case of persistent, painless hemospermia in a young Caucasian caused by metastatic malignant melanoma of the right seminal vesicle. The diagnosis was established by magnetic resonance imaging and transrectal ultrasound-guided biopsy. In the subsequent diagnostic workup the primary location of the tumor remained unknown but concomitant pulmonary, hepatic and supraclavicular lymph node metastases have been detected. Despite immediate chemotherapy initiation the patient finally succumbed to his progressive disease six months later.
Conclusions
Malignant melanoma should be considered as a rare differential diagnosis of hemospermia after common causes have been ruled out.
Background
The epidural route is still considered the gold standard for labour analgesia, although it is not without serious consequences when incorrect placement goes unrecognized, e.g. in case of intravascular, intrathecal and subdural placements. Until now there has not been a viable alternative to epidural analgesia especially in view of the neonatal outcome and the need for respiratory support when long-acting opioids are used via the parenteral route. Pethidine and meptazinol are far from ideal having been described as providing rather sedation than analgesia, affecting the cardiotocograph (CTG), causing fetal acidosis and having active metabolites with prolonged half-lives especially in the neonate. Despite these obvious shortcomings, intramuscular and intravenously administered pethidine and comparable substances are still frequently used in delivery units.
Since the end of the 90ths remifentanil administered in a patient-controlled mode (PCA) had been reported as a useful alternative for labour analgesia in those women who either don’t want, can’t have or don’t need epidural analgesia.
Discussion
In view of the need for conversion to central neuraxial blocks and the analgesic effect remifentanil has been demonstrated to be superior to pethidine. Despite being less effective in terms of the resulting pain scores, clinical studies suggest that the satisfaction with analgesia may be comparable to that obtained with epidural analgesia. Owing to this fact, remifentanil has gained a place in modern labour analgesia in many institutions.
However, the fact that remifentanil may cause harm should not be forgotten when the use of this potent mu-agonist is considered for the use in labouring women. In the setting of one-to-one midwifery care, appropriate monitoring and providing that enough experience exists with this potent opioid and the treatment of potential complications, remifentanil PCA is a useful option in addition to epidural analgesia and other central neuraxial blocks. Already described serious consequences should remind us not refer to remifentanil PCA as a “poor man’s epidural” and to safely administer remifentanil with an appropriate indication.
Summary
Therefore, the authors conclude that economic considerations and potential cost-savings in conjunction with remifentanil PCA may not be appropriate main endpoints when studying this valuable method for labour analgesia.
The Chaco leaf-cutting ant Atta vollenweideri (Forel) inhabits large and deep subterranean nests composed of a large number of fungus and refuse chambers. The ants dispose of the excavated soil by forming small pellets that are carried to the surface. For ants in general, the organisation of underground soil transport during nest building remains completely unknown. In the laboratory, we investigated how soil pellets are formed and transported, and whether their occurrence influences the spatial organisation of collective digging. Similar to leaf transport, we discovered size matching between soil pellet mass and carrier mass. Workers observed while digging excavated pellets at a rate of 26 per hour. Each excavator deposited its pellets in an individual cluster, independently of the preferred deposition sites of other excavators. Soil pellets were transported sequentially over 2 m, and the transport involved up to 12 workers belonging to three functionally distinct groups: excavators, several short-distance carriers that dropped the collected pellets after a few centimetres, and long-distance, last carriers that reached the final deposition site. When initiating a new excavation, the proportion of long-distance carriers increased from 18% to 45% within the first five hours, and remained unchanged over more than 20 hours. Accumulated, freshly-excavated pellets significantly influenced the workers' decision where to start digging in a choice experiment. Thus, pellets temporarily accumulated as a result of their sequential transport provide cues that spatially organise collective nest excavation.
Background
The knowledge of metabolic pathways and fluxes is important to understand the adaptation of organisms to their biotic and abiotic environment. The specific distribution of stable isotope labelled precursors into metabolic products can be taken as fingerprints of the metabolic events and dynamics through the metabolic networks. An open-source software is required that easily and rapidly calculates from mass spectra of labelled metabolites, derivatives and their fragments global isotope excess and isotopomer distribution.
Results
The open-source software “Least Square Mass Isotopomer Analyzer” (LS-MIDA) is presented that processes experimental mass spectrometry (MS) data on the basis of metabolite information such as the number of atoms in the compound, mass to charge ratio (m/e or m/z) values of the compounds and fragments under study, and the experimental relative MS intensities reflecting the enrichments of isotopomers in 13C- or 15 N-labelled compounds, in comparison to the natural abundances in the unlabelled molecules. The software uses Brauman’s least square method of linear regression. As a result, global isotope enrichments of the metabolite or fragment under study and the molar abundances of each isotopomer are obtained and displayed.
Conclusions
The new software provides an open-source platform that easily and rapidly converts experimental MS patterns of labelled metabolites into isotopomer enrichments that are the basis for subsequent observation-driven analysis of pathways and fluxes, as well as for model-driven metabolic flux calculations.
Introduction
Groin infections resulting in arterial bleeding due to bacterial vessel destruction are a severe challenge in vascular surgery. Patients with them most often present as emergencies and therefore need individualized reconstruction solutions.
Case presentation
Case 1 is a 67-year-old man with infectious bleeding after an autologous reconstruction of the femoral bifurcation with greater saphenous vein due to infection of a bovine pericard patch after thrombendarterectomy. Case 2 is a 35-year-old male drug addict and had severe femoral bleeding and infection after repeated intravenous and intra-arterial substance abuse. Both patients were treated with an autologous obturator bypass of the superficial femoral vein. We review the current literature and highlight our therapeutic concept of this clinical entity.
Conclusions
Treatment should include systemic antibiotic medication, surgical control of the infectious site, revascularization and soft tissue repair. An extra-anatomical obturator bypass with autologous superficial femoral vein should be considered as the safest revascularization procedure in infections caused by highly pathogenic bacteria.
The ability to perceive the number of objects has been known to exist in vertebrates for a few decades, but recent behavioral investigations have demonstrated that several invertebrate species can also be placed on the continuum of numerical abilities shared with birds, mammals, and reptiles. In this review article, we present the main experimental studies that have examined the ability of insects to use numerical information. These studies have made use of a wide range of methodologies, and for this reason it is striking that a common finding is the inability of the tested animals to discriminate numerical quantities greater than four. Furthermore, the finding that bees can not only transfer learnt numerical discrimination to novel objects, but also to novel numerosities, is strongly suggestive of a true, albeit limited, ability to count. Later in the review, we evaluate the available evidence to narrow down the possible mechanisms that the animals might be using to solve the number-based experimental tasks presented to them. We conclude by suggesting avenues of further research that take into account variables such as the animals’ age and experience, as well as complementary cognitive systems such as attention and the time sense.
Background
Cataract and glaucoma are both common comorbidities among older patients. Combining glaucoma surgery with minimal invasive phacoemulsification (phaco) is a considerable option to treat both conditions at the same time, although the combination with filtration surgery can produce a strong inflammatory response. Combined non-penetrating procedures like canaloplasty have shown to reduce intraocular pressure (IOP) comparable to trabeculectomy without the risk of serious bleb-related complications. The purpose of this retrospective study was to compare the outcomes of phacotrabeculectomy and phacocanaloplasty.
Methods
Thirty-nine eyes with concomitant cataract and glaucoma who underwent phacotrabeculectomy (n = 20; 51.3%) or phacocanaloplasty (n = 19; 48.7%) were included into this trial on reduction of IOP, use of medication, success rate, incidence of complications and postsurgical interventions. Complete success was defined as IOP reduction by 30% or more and to 21 mmHg or less (definition 1a) or IOP to less than 18 mmHg (definition 2a) without glaucoma medication.
Results
Over a 12-month follow-up, baseline IOP significantly decreased from 30.0 ± 5.3 mmHg with a mean of 2.5 ± 1.2 glaucoma medications to 11.7 ± 3.5 mmHg with a mean of 0.2 ± 0.4 medications in eyes with phacotrabeculectomy (P < .0001). Eyes with phacocanaloplasty had a preoperative IOP of 28.3 ± 4.1 mmHg and were on 2.8 ± 1.1 IOP-lowering drugs. At 12 months, IOP significantly decreased to 12.6 ± 2.1 mmHg and less glaucoma medications were necessary (mean 1.0 ± 1.5 topical medications; P < .05). 15 patients (78.9%) with phacotrabeculectomy and 9 patients (60.0%) in the phacocanaloplasty group showed complete success according to definition 1 and 2 after 1 year (P = .276). Postsurgical complications were seen in 7 patients (36.8%) of the phacocanaloplasty group which included intraoperative macroperforation of the trabeculo-Descemet membrane (5.3%), hyphema (21.1%) and bleb formation (10.5%). Although more complications were observed in the phacotrabeculectomy group, no statistically significant difference was found.
Conclusions
Phacocanaloplasty offers a new alternative to phacotrabeculectomy for treatment of concomitant glaucoma and cataract, although phacotrabeculectomy yielded in better results in terms of IOP maintained without glaucoma medications.
The conserved, ubiquitin-selective AAA ATPase Cdc48 regulates numerous cellular processes including protein quality control, DNA repair and the cell cycle. Cdc48 function is tightly controlled by a multitude of cofactors mediating substrate specificity and processing. The UBX domain protein Shp1 is a bona fide substrate-recruiting cofactor of Cdc48 in the budding yeast S. cerevisiae. Even though Shp1 has been proposed to be a positive regulator of Glc7, the catalytic subunit of protein phosphatase 1 in S. cerevisiae, its cellular functions in complex with Cdc48 remain largely unknown. Here we show that deletion of the SHP1 gene results in severe growth defects and a cell cycle delay at the metaphase to anaphase transition caused by reduced Glc7 activity. Using an engineered Cdc48 binding-deficient variant of Shp1, we establish the Cdc48Shp1 complex as a critical regulator of mitotic Glc7 activity. We demonstrate that shp1 mutants possess a perturbed balance of Glc7 phosphatase and Ipl1 (Aurora B) kinase activities and show that hyper-phosphorylation of the kinetochore protein Dam1, a key mitotic substrate of Glc7 and Ipl1, is a critical defect in shp1. We also show for the first time a physical interaction between Glc7 and Shp1 in vivo. Whereas loss of Shp1 does not significantly affect Glc7 protein levels or localization, it causes reduced binding of the activator protein Glc8 to Glc7. Our data suggest that the Cdc48Shp1 complex controls Glc7 activity by regulating its interaction with Glc8 and possibly further regulatory subunits.
Background
During reverse transcription, retroviruses duplicate the long terminal repeats (LTRs). These identical LTRs carry both promoter regions and functional polyadenylation sites. To express full-length transcripts, retroviruses have to suppress polyadenylation in the 5′LTR and activate polyadenylation in the 3′LTR. Foamy viruses have a unique LTR structure with respect to the location of the major splice donor (MSD), which is located upstream of the polyadenylation signal.
Results
Here, we describe the mechanisms of foamy viruses regulating polyadenylation. We show that binding of the U1 small nuclear ribonucleoprotein (U1snRNP) to the MSD suppresses polyadenylation at the 5′LTR. In contrast, polyadenylation at the 3′LTR is achieved by adoption of a different RNA structure at the MSD region, which blocks U1snRNP binding and furthers RNA cleavage and subsequent polyadenylation.
Conclusion
Recently, it was shown that U1snRNP is able to suppress the usage of intronic cryptic polyadenylation sites in the cellular genome. Foamy viruses take advantage of this surveillance mechanism to suppress premature polyadenylation at the 5’end of their RNA. At the 3’end, Foamy viruses use a secondary structure to presumably block access of U1snRNP and thereby activate polyadenylation at the end of the genome. Our data reveal a contribution of U1snRNP to cellular polyadenylation site selection and to the regulation of gene expression.
Background
Published models predicting nasal colonization with Methicillin-resistant Staphylococcus aureus among hospital admissions predominantly focus on separation of carriers from non-carriers and are frequently evaluated using measures of discrimination. In contrast, accurate estimation of carriage probability, which may inform decisions regarding treatment and infection control, is rarely assessed. Furthermore, no published models adjust for MRSA prevalence.
Methods
Using logistic regression, a scoring system (values from 0 to 200) predicting nasal carriage of MRSA was created using a derivation cohort of 3091 individuals admitted to a European tertiary referral center between July 2007 and March 2008. The expected positive predictive value of a rapid diagnostic test (GeneOhm, Becton & Dickinson Co.) was modeled using non-linear regression according to score. Models were validated on a second cohort from the same hospital consisting of 2043 patients admitted between August 2008 and January 2012. Our suggested correction score for prevalence was proportional to the log-transformed odds ratio between cohorts. Calibration before and after correction, i.e. accurate classification into arbitrary strata, was assessed with the Hosmer-Lemeshow-Test.
Results
Treating culture as reference, the rapid diagnostic test had positive predictive values of 64.8% and 54.0% in derivation and internal validation corhorts with prevalences of 2.3% and 1.7%, respectively. In addition to low prevalence, low positive predictive values were due to high proportion (> 66%) of mecA-negative Staphylococcus aureus among false positive results. Age, nursing home residence, admission through the medical emergency department, and ICD-10-GM admission diagnoses starting with “A” or “J” were associated with MRSA carriage and were thus included in the scoring system, which showed good calibration in predicting probability of carriage and the rapid diagnostic test’s expected positive predictive value. Calibration for both probability of carriage and expected positive predictive value in the internal validation cohort was improved by applying the correction score.
Conclusions
Given a set of patient parameters, the presented models accurately predict a) probability of nasal carriage of MRSA and b) a rapid diagnostic test’s expected positive predictive value. While the former can inform decisions regarding empiric antibiotic treatment and infection control, the latter can influence choice of screening method.
Background
High expression of constitutive histone γ-H2AX, a sensitive marker of DNA damage, might be indicative of defective DNA repair pathway or genomic instability. 53BP1 (p53-binding protein 1) is a conserved checkpoint protein with properties of a DNA double-strand breaks sensor. This study explores the relationship between the clinical radiosensitivity of tumor patients and the expression/induction of γ-H2AX and 53BP1 in vitro.
Methods
Using immunostaining, we assessed spontaneous and radiation-induced foci of γ-H2AX and 53 BP1 in peripheral blood mononuclear cells derived from unselected breast cancer (BC) patients (n=57) undergoing radiotherapy (RT). Cells from apparently healthy donors (n=12) served as references.
Results
Non-irradiated cells from controls and unselected BC patients exhibited similar baseline levels of DNA damage assessed by γ-H2AX and 53BP1 foci. At the same time, the γ-H2AX assay of in vitro irradiated cells revealed significant differences between the control group and the group of unselected BC patients with respect to the initial (0.5 Gy, 30 min) and residual (2 Gy, 24 h post-radiation) DNA damage. The numbers of 53BP1 foci analyzed in 35 BC patients were significantly higher than in controls only in case of residual DNA damage. A weak correlation was found between residual foci of both proteins tested. In addition, cells from cancer patients with an adverse acute skin reaction (grade 3) to RT showed significantly increased radiation-induced γ-H2AX foci and their protracted disappearance compared to the group of BC patients with normal skin reaction (grade 0–1). The mean number of γ-H2AX foci after 5 clinical fractions was significantly higher than that before RT, especially in clinically radiosensitive patients.
Conclusions
The γ-H2AX assay may have potential for screening individual radiosensitivity of breast cancer patients.
Background: Desmoplastic fibroma (DF) is an extremely rare locally aggressive bone tumor with an incidence of 0.11% of all primary bone tumors. The typical clinical presentation is pain and swelling above the affected area. The most common sites of involvement are the mandible and the metaphysis of long bones. Histologically and biologically, desmoplastic fibroma mimics extra-abdominal desmoid tumor of soft tissue.
Case Presentation and Literature Review: A case of a 27-year old man with DF in the ilium, including the clinical, radiological and histological findings over a 4-year period is presented here. CT scans performed in 3-year intervals prior to surgical intervention were compared with respect to tumor extension and cortical breakthrough. The patient was treated with curettage and grafting based on anatomical considerations. Follow-up CT scans over 18-months are also documented here. Additionally, a review and analysis of 271 cases including the presented case with particular emphasis on imaging patterns in MRI and CT as well as treatment modalities and outcomes are presented.
Conclusion: In patients with desmoplastic fibroma, CT is the preferred imaging technique for both the diagnosis of intraosseus tumor extension and assessment of cortical involvement, whereas MRI is favored for the assessment of extraosseus tumor growth and preoperative planning. While tumor resection remains the preferred treatment for DF, curettage and grafting prove to be an acceptable alternative treatment modality with close follow-up when resection is not
possible. Curettage and grafting have been shown to provide good clinical results and are associated with long recurrence free intervals.
Coherent two-dimensional electronic spectroscopy in the Soret band of a chiral porphyrin dimer

Using coherent two-dimensional (2D) electronic spectroscopy in fully noncollinear geometry, we observe the excitonic coupling of β,β'-linked bis[tetraphenylporphyrinato-zinc(II)] on an ultrafast timescale in the excited state. The results for two states in the Soret band originating from an excitonic splitting are explained by population transfer with approximately 100 fs from the energetically higher to the lower excitonic state. This interpretation is consistent with exemplary calculations of 2D spectra for a model four-level system with coupling.
Background
The aim of the study was to evaluate the effects of universal mass vaccination (UMV) against rotavirus (RV) on the hospitalization rates, nosocomial RV infections and RV-gastroenteritis (GE)-associated secondary blood stream infections (BSI).
Methods
The retrospective evaluation (2002–2009) by chart analysis included all clinically diagnosed and microbiologically confirmed RV-GE cases in a large tertiary care hospital in Austria. The pre-vaccination period (2002–2005) was compared with the recommended and early funded (2006–2007) and the funded (2008–2009) vaccination periods. Primary outcomes were RV-GE-associated hospitalizations, secondary outcomes nosocomial RV disease, secondary BSI and direct hospitalization costs for children and their accompanying persons.
Results
In 1,532 children with RV-GE, a significant reduction by 73.9% of hospitalized RV-GE cases per year could be observed between the pre-vaccination and the funded vaccination period, which was most pronounced in the age groups 0–11 months (by 87.8%), 6–10 years (by 84.2%) and 11–18 years (88.9%). In the funded vaccination period, a reduction by 71.9% of nosocomial RV-GE cases per year was found compared to the pre-vaccination period. Fatalities due to nosocomial RV-GE were only observed in the pre-vaccination period (3 cases). Direct costs of hospitalized, community-acquired RV-GE cases per year were reduced by 72.7% in the funded vaccination period. The reduction of direct costs for patients (by 86.9%) and accompanying persons (86.2%) was most pronounced in the age group 0–11 months.
Conclusions
UMV may have contributed to the significant decrease of RV-GE-associated hospitalizations, to a reduction in nosocomial RV infections and RV-associated morbidity due to secondary BSI and reduced direct hospitalization costs. The reduction in nosocomial cases is an important aspect considering severe disease courses in hospitalized patients with co-morbidities and death due to nosocomial RV-GE.
Dialysepatienten weisen eine hohe Anzahl kardiovaskulärer Ereignisse auf. Betrachtet man die häufigsten Todesursachen von Dialysepatienten, so fällt ein großer Teil in den kardiovaskulären Bereich. In dieser Arbeit wurde der Einfluss von Aldosteron und Cortisol auf kardiale und vaskuläre Ereignisse bei Dialysepatienten mit Diabetes mellitus untersucht. Dazu wurden Daten von 1255 Dialysepatienten mit Diabetes mellitus aus der Deutschen Diabetes Dialyse Studie analysiert.
In der vorliegenden Arbeit konnte gezeigt werden, dass mit erhöhten Aldosteronkonzentrationen ein signifikanter Anstieg des Risikos für plötzlichen Herztod (HR: 1.69; 95% CI: 1.06–2.69) einhergeht. Das Risiko an plötzlichem Herztod zu versterben war bei hohen Konzentrationen von Aldosteron und gleichzeitig vorliegenden hohen Konzentrationen von Cortisol noch deutlicher erhöht (HR: 2.86, 95% CI: 1.32–6.21). Ebenso war die Gesamtsterblichkeit signifikant erhöht bei Patienten, die hohe Aldosteron- und Cortisolkonzentrationen aufwiesen im Vergleich zu Patienten mit niedrigen Spiegeln beider Hormone (HR: 1.62, 95% CI: 1.01–2.62).
In dieser Arbeit konnte somit ein deutlicher Zusammenhang hoher Aldosteron- und Cortisolkonzentrationen mit plötzlichem Herztod und Gesamtsterblichkeit gezeigt werden.
Background
Occupational exposure to live meningococci can potentially cause invasive meningococcal disease in laboratory staff. While, until recently, immunization with quadrivalent polysaccharide vaccine represented one cornerstone of protection, data on long-term persistence of antibodies in adults remain scarce.
Methods
We analyzed the relationship of antibody levels and time following quadrivalent polysaccharide vaccination (Mencevax® ACWY, GlaxoSmithKline) in a cross-sectional sample of 20 laboratory workers vaccinated at ages between 16.4 to 40.7 years from Germany. Sera were obtained 0.4 to 158.5 (median 35.3) months after vaccination. At the time of sampling, laboratory workers had been regularly exposed to meningococci for periods between 3.2 to 163.8 (median 41.2) months. Serum bactericidal assay (SBA) with rabbit complement and a microsphere-based flow analysis method were used to determine bactericidal titers and concentrations of IgG, respectively, against serogroups A, C, W135, and Y. Decay of antibodies was modeled using linear regression. Protective levels were defined as SBA titers ≥ 8.
Results
Half-lives of SBA titers against serogroups A, C, W135, and Y were estimated at 27.4, 21.9, 18.8, and 28.0 months, respectively. Average durations of protection were estimated at 183.9, 182.0, 114.6, and 216.4 months, respectively. Inter-individual variation was high; using lower margins of 95% prediction intervals, minimal durations of protection against serogroups A, C, W135 and Y were estimated at 33.5, 24.6, 0.0, and 55.1 months, respectively. The proportion of staff with protective SBA titers against W135 (65.0%) was significantly lower than proportions protected against A (95.0%), C (94.7%), and Y (95.0%). Consistently, geometric mean titer (97.0) and geometric mean concentration of IgG (2.1 μg/ml) was lowest against serogroup W135. SBA titers in a subset of individuals with incomplete protection rose to ≥ 128 (≥ 8 fold) after reimmunization with a quadrivalent glycoconjugate vaccine.
Conclusions
The average duration of protection following immunization with a quadrivalent polysaccharide vaccine in adults was ≥ 115 months regardless of serogroup. A substantial proportion (approximately 23% according to our decay model) of adult vaccinees may not retain protection against serogroup W135 for five years, the time suggested for reimmunization.
Background
Laxatives are among the most widely used over-the-counter medications in the United States but studies examining their potential hazardous side effects are sparse. Associations between laxative use and risk for fractures and change in bone mineral density [BMD] have not previously been investigated.
Methods
This prospective analysis included 161,808 postmenopausal women (8907 users and 151,497 nonusers of laxatives) enrolled in the WHI Observational Study and Clinical Trials. Women were recruited from October 1, 1993, to December 31, 1998, at 40 clinical centers in the United States and were eligible if they were 50 to 79 years old and were postmenopausal at the time of enrollment. Medication inventories were obtained during in-person interviews at baseline and at the 3-year follow-up visit on everyone. Data on self-reported falls (≥2), fractures (hip and total fractures) were used. BMD was determined at baseline and year 3 at 3 of the 40 clinical centers of the WHI.
Results
Age-adjusted rates of hip fractures and total fractures, but not for falls were similar between laxative users and non-users regardless of duration of laxative use. The multivariate-adjusted hazard ratios for any laxative use were 1.06 (95% confidence interval [CI], 1.03-1.10) for falls, 1.02 (95% CI, 0.85-1.22) for hip fractures and 1.01 (95% CI, 0.96-1.07) for total fractures. The BMD levels did not statistically differ between laxative users and nonusers at any skeletal site after 3-years intake.
Conclusion
These findings support a modest association between laxative use and increase in the risk of falls but not for fractures. Its use did not decrease bone mineral density levels in postmenopausal women. Maintaining physical functioning, and providing adequate treatment of comorbidities that predispose individuals for falls should be considered as first measures to avoid potential negative consequences associated with laxative use.
Background
Malignant pleural effusion (MPE) is associated with advanced stages of lung cancer and is mainly dependent on invasion of the pleura and expression of vascular endothelial growth factor (VEGF) by cancer cells. As MPE indicates an incurable disease with limited palliative treatment options and poor outcome, there is an urgent need for new and efficient treatment options.
Methods
In this study, we used subcutaneously generated PC14PE6 lung adenocarcinoma xenografts in athymic mice that developed subcutaneous malignant effusions (ME) which mimic pleural effusions of the orthotopic model. Using this approach monitoring of therapeutic intervention was facilitated by direct observation of subcutaneous ME formation without the need of sacrificing mice or special imaging equipment as in case of MPE. Further, we tested oncolytic virotherapy using Vaccinia virus as a novel treatment modality against ME in this subcutaneous PC14PE6 xenograft model of advanced lung adenocarcinoma.
Results
We demonstrated significant therapeutic efficacy of Vaccinia virus treatment of both advanced lung adenocarcinoma and tumor-associated ME. We attribute the efficacy to the virus-mediated reduction of tumor cell-derived VEGF levels in tumors, decreased invasion of tumor cells into the peritumoral tissue, and to viral infection of the blood vessel-invading tumor cells. Moreover, we showed that the use of oncolytic Vaccinia virus encoding for a single-chain antibody (scAb) against VEGF (GLAF-1) significantly enhanced mono-therapy of oncolytic treatment.
Conclusions
Here, we demonstrate for the first time that oncolytic virotherapy using tumor-specific Vaccinia virus represents a novel and promising treatment modality for therapy of ME associated with advanced lung cancer.
Die durch Polyethylenabrieb bedingte Osteolyse trägt wesentlich zur Lockerung der Hüftgelenkendoprothese bei. Das Ziel der vorliegenden retrospektiven Studie war, den Einfluß der Patientenaktivität auf den Polyethylenabrieb zu bestimmen.
Es wurden 104 gut funktionierende primär implantierte Hüftgelenkprothesen [HG®-Pfanne, konventionelles PE, 28 mm Keramikköpfe] mittels Abriebmessungen [HipAnalysisSuite®Version 8.0.4.3] an 1203 Röntgenaufnahmen über durchschnittlich 12 Jahre untersucht.
Die Aktivität der durchschnittlich 58-jährigen Patienten wurde mit speziellen Fragebögen und einem Accelerometer bestimmt. Die Hochrechnung auf ein Jahr ergab 1,8 Millionen Lastwechsel im Durchschnitt und durchschnittlich 17 Stunden Aktivität pro Tag. Dabei zeigten die Messinstrumente HHS, Aktivitätseinschätzung
durch den Arzt, Gehstrecke, UCLA und TWB ansteigende Korrelationen zu den durchschnittlichen absoluten Lastwechseln pro Tag. Es zeigte sich eine durchschnittliche jährliche lineare Abriebrate von 0,1 mm, die zu 18 % von der Aktivität beeinflußt wurde und eine jährliche volumetrische Abriebrate von durchschnittlich 53 mm³, die zu 19 % von den durchgeführten Lastwechseln beeinflußt wurde. Abriebrate. Für die durchschnittlichen Lastwechsel im mittleren Aktivitätsniveau und die absoluten Lastwechsel in Kombination mit dem PeakActivityIndex ermittelte sich eine Prädiktion für das Risiko einer jährlichen linearen Abriebrate von mehr als 0,15 mm von 78%.
Diese Langzeitstudie etabliert eine direkte Korrelation zwischen der Patientenaktivität und dem Polyethylenabrieb und die untersuchte Gleitpaarung zeigte gute Langzeitergebnisse bei durchschnittlicher Aktivität berenteter Patienten. Die dem Operateur zur Verfügung
stehenden Messinstrumente, um die Patientenaktivität einzuschätzen, sind allenfalls begrenzt geeignet, die Patientenaktivität zu beurteilen. Der Operateur kann den Einfluss der Patientenaktivität auf die jährliche Abriebrate nur durch die Wahl der bestmöglichen Gleitpaarung minimieren.
Mutationen in Genen, die für Kernhüllproteine codieren sind mit einer stetig zunehmenden Anzahl menschlicher Erkrankungen verbunden, die als Envelopathien bezeichnet werden. Erstaunlicherweise betrifft die Pathologie dieser Krankheiten spezifische Gewebe und Organe, obwohl entsprechende Proteine meist ubiquitär exprimiert werden. So führen beispielsweise Defekte in Emerin, einem Protein der inneren Kernhülle, zur X-chromosomalen Emery-
Dreifuss Muskeldystrophie (EDMD). Diese Krankheit ist durch Muskelschwäche oder –
schwund gekennzeichnet. Defekte im Kernhüllprotein MAN1 sind dagegen mit Krankheiten verbunden, die Knochen- und Hautgewebe betreffen. Interessanterweise besitzen beide
Proteine eine evolutionär hoch konservierte Domäne, die sog. LEM-Domäne. LEM-Domänen Proteine können mit der Kernlamina interagieren, ebenso mit dem sog. Barrier-to-
Autointegration Factor (BAF) sowie mit zahlreichen Transkriptionsfaktoren. Dennoch ist die
funktionelle Rolle der LEM-Domänen Proteine bis dato nicht vollständig aufgeklärt. In der vorliegenden Studie sollten daher die Funktionen von MAN1 und Emerin während der Frühentwicklung von Xenopus laevis untersucht werden.
Vorangehende Untersuchungen zeigten, dass Mikroinjektionen von XMAN1-
Antikörpern in Zwei-Zell-Stadien befruchteter Eizellen zu einem Arrest der Zellteilung in der injizierten Blastomere führten. Da dabei eine Störung der Kernhüllbildung spekuliert wurde, sollte durch Antikörper-vermittelter Inhibition von XMAN1 die Bildung von in vitro Kernen im Xenopus Eiextrakt untersucht werden. Dabei wurden Kerne beobachtet, die dekondensiertes
Chromatin zeigten, bei denen jedoch eine Fusion von Membranvesikeln zu einer durchgehenden Kernhülle nicht stattgefunden hatte. Frühere Charakterisierungen von MAN1 und Emerin zeigten unterschiedliche Expressionsmuster während der Entwicklung von X. laevis. Da XMAN1 ubiquitär exprimiert und Xemerin jedoch erstmals ab Stadium 41 nachweisbar ist, war es mittels Mikroinjektion von
Xemerin möglich zu zeigen, dass es in der Lage ist den Arrest der Zellteilung zu verhindern. Es wurde daher die These aufgestellt, dass MAN1 und Emerin während der Frühentwicklung von Xenopus überlappende Funktionen besitzen. Um diese These zu prüfen, wurde zunächst
unter Verwendung des Proximity Ligation Assays untersucht, ob beide Proteine miteinander interagieren können. Mit Hilfe dieser Methode konnte gezeigt werden, dass Interaktionen beider Proteine innerhalb der Kernhülle lokalisieren. Die Bcl-2-Familienmitglieder A1 und sein humanes Homolog Bfl-1 gewährleisten das Überleben der Zelle. Gleichzeitig trägt eine Dysregulation der Expression von A1/ Bfl-1 zur Krebsentstehung bei. Die Stabilität von A1/ Bfl-1 wird durch deren Ubiquitinylierung sowie die anschließende proteosomale Degradation gesteuert. Mit Hilfe eines Yeast-Two-Hybrid-Screens wurde die E3-Ubiquitinligase HectD1 als potentieller Interaktionspartner von A1/ Bfl-1 identifiziert. Die Interaktion von A1 und HectD1 des Yeast-Two-Hybrid-Screens konnte in Säugerzellen bestätigt werden. Desweiteren konnte gezeigt werden, dass lediglich 87 Aminosäuren für eine Interaktion von HectD1 und A1 nötig sind. Da membrangebundenes HectD1 zu einer Translokation von zytosolischem A1 an die Zellmembran führt, kann man davon ausgehen, dass beide Proteine auch in vivo miteinander interagieren. Eine dominant negative HectD1-Mutante schließlich beeinflusst die Ubiqutinylierung von A1 und führt somit zu dessen Stabilisierung. Diese Daten legen nahe, dass HectD1 ein wichtiger negativer Regulator von A1/ Bfl-1 ist und dass HectD1 für die Regulierung der A1/ Bfl-1-Proteinmenge in (Krebs)zellen sehr wichtig ist.
Atherosclerosis is accepted to be a chronic inflammatory disease of the arterial vessel wall. Several cellular subsets of the immune system are involved in its initiation and progression, such as monocytes, macrophages, T and B cells. Recent research has demonstrated that dendritic cells (DCs) contribute to atherosclerosis, too. DCs are defined by their ability to sense and phagocyte antigens, to migrate and to prime other immune cells, such as T cells. Although all DCs share these functional characteristics, they are heterogeneous with respect to phenotype and origin. Several markers have been used to describe DCs in different lymphoid and non-lymphoid organs; however, none of them has proven to be unambiguous. The expression of surface molecules is highly variable depending on the state of activation and the surrounding tissue. Furthermore, DCs in the aorta or the atherosclerotic plaque can be derived from designated precursor cells or from monocytes. In addition, DCs share both their marker expression and their functional characteristics with other myeloid cells like monocytes and macrophages. The repertoire of aortic DCs in healthy and atherosclerotic mice has just recently started to be explored, but yet there is no systemic study available, which describes the aortic DC compartment. Because it is conceivable that distinct aortic DC subsets exert dedicated functions, a detailed description of vascular DCs is required. The first part of this thesis characterizes DC subsets in healthy and atherosclerotic mice. It describes a previously unrecognized DC subset and also sheds light on the origin of vascular DCs. In recent years, microRNAs (miRNAs) have been demonstrated to regulate several cellular functions, such as apoptosis, differentiation, development or proliferation. Although several cell types have been characterized extensively with regard to the miRNAs involved in their regulation, only few studies are available that focus on the role of miRNAs in DCs. Because an improved understanding of the regulation of DC functions would allow for new therapeutic options, research on miRNAs in DCs is required. The second part of this thesis focuses on the role of the miRNA cluster miR- 17~92 in DCs by exploring its functions in healthy and atherosclerotic mice. This thesis clearly demonstrates for the first time an anti-inflammatory and atheroprotective role for the miR17-92 cluster. A model for its mechanism is suggested.
Using viruses to treat cancer is a novel approach to an age-old disease. Oncolytic viruses are native or recombinant viruses that have the innate or enhanced capability to infect tumour cells, replicate within the tumour microenvironment and subsequently lyse those cells. One representative, the vaccinia virus (VACV), belongs to the orthopoxvirus genus of the Poxviridae family. GLV-1h68, a recombinant and attenuated vaccinia virus devel- oped by the Genelux Corporation, is a member of this family currently being tested in various phase I/II clinical trials under the name GL-ONC1. It has been shown to specif- ically replicate in tumour cells while sparing healthy tissue and to metabolise prodrug at or transport immunological payloads to the site of affliction. Since imaging modalities offer little insight into viral replication deep within the body, and because oncolytic virotherapy is dependent on replication within the target tissue, the need for a monitoring system is evident. Pharmacokinetic analysis of this oncolytic agent was to give insight into the dynamics present in tumours during treatment. This, in turn, would give clinicians the opportunity to monitor the efficacy as early as possible after the onset of treatment, to observe treatment progression and possibly to gauge prognosis, without resorting to invasive procedures, e.g. biopsies. A criteria for viable biomarkers was that it had to be directly dependent on viral replica- tion. Ideally, a marker for treatment efficacy would be specific to the treatment modality, not necessarily the treatment type. Such a marker would be highly detectable (high sen- sitivity), specific for the treatment (high specificity), and present in an easily obtained specimen (blood). Taking this into consideration, the biomarkers were chosen for their potential to be indicators of viral replication. Thus, the biomarkers analysed in this thesis are: the native proteins expressed by the viral genes A27L and B5R, the virally encoded recombinant proteins β-galactosidase, β-glucuronidase, green fluorescent protein (GFP), carboxypeptidase G2 (CPG2) and carcinoembryonic antigen (CEA). Each marker is under the control of one of five different promoters present. All recombinant viruses used in this thesis express A27L, B5R, GFP and β-glucuronidase and all are derived from the parental virus GLV-1h68.
