Hintergrund: Seit Beginn der 90er Jahre wird Botulinumtoxin A zunehmend zur Behandlung spastischer Spitzfüße im Kindesalter eingesetzt. Eine genaue Beurteilung des Gangbildes bei Zerebralparesen war bisher nur durch aufwendige Verfahren möglich. Es wurde ein videogestütztes Verfahren zur Ganganalyse bei der Therapieevaluation des spastischen Spitzfußes entwickelt. Patienten und Methode: Bei 13 Patienten mit spastischem Spitzfuß wurde das Gangbild in standardisierter Form mit der Videokamera aufgezeichnet. Insgesamt 76 Videosequenzen wurden von 2 Untersuchern unabhängig voneinander beurteilt. Die Reliabilität wurde anhand des Kappa- Wertes für jeden Beurteilungsparameter überprüft. Ergebnisse: 16 Kriterien zur Beurteilung des Gangbilds wurden entwickelt. Die Kappa-Werte für die einzelnen Kriterien lagen zwischen 0,64 und 1, d.h. die Reliabilität war gut bis sehr gut. Schlußfolgerungen: Vorteil dieses Verfahrens gegenüber früheren Versuchen einer videogestützten Ganganalyse ist die hohe Reliabilität bei präziser Definition der Items. Diese Methode könnte deshalb besonders hilfreich für die Therapieevaluation des spastischen Spitzfußes sein.
Background
Reliable central venous access (CVC) is essential for hematology–oncology patients since frequent puncture of peripheral veins—e.g., for chemotherapy, antibiotic administration, repeated blood sampling, and monitoring—can cause unacceptable pain and psychological trauma, as well as severe side effects in cases of extravasation of chemotherapy drugs. However, CVC lines still carry major risk factors, including thrombosis, infection (e.g., entry site, tunnel, and luminal infections), and catheter dislocation, leakage, or breakage.
Methods
Here we performed a retrospective database analysis to determine the incidence of CVC-associated thrombosis in a single-center cohort of 448 pediatric oncologic patients, and to analyze whether any subgroup of patients was at increased risk and thus might benefit from prophylactic anticoagulation.
Results
Of the 448 patients, 269 consecutive patients received a CVC, and 55 of these 269 patients (20%) also had a thrombosis. Of these 55 patients, 43 had at least one CVC-associated thrombosis (total number of CVC-associated thrombosis: n = 52). Among all patients, the median duration of CVC exposure was 464 days. Regarding exposure time, no significant difference was found between patients with and without CVC-associated thrombosis. Subclavia catheters and advanced tumor stages seem to be the main risk factors for the development of CVC-associated thrombosis, whereas pharmacologic prophylaxis did not seem to have a relevant impact on the rate of thrombosis.
Conclusions
We conclude that pediatric surgeons and oncologists should pay close attention to ensuring optimal and accurate CVC placement, as this appears the most effective tool to minimize CVC-associated complications.
Varicella-zoster virus infections in immunocompromised patients - a single centre 6-years analysis

Background: Infection with varicella-zoster virus (VZV) contemporaneously with malignant disease or immunosuppression represents a particular challenge and requires individualized decisions and treatment. Although the increasing use of varicella-vaccines in the general population and rapid initiation of VZVimmunoglobulins and acyclovir in case of exposure has been beneficial for some patients, immunocompromised individuals are still at risk for unfavourable courses. Methods: In this single center, 6-year analysis we review incidence, hospitalization and complication rates of VZVinfections in our center and compare them to published data. Furthermore, we report three instructive cases. Results: Hospitalization rate of referred children with VZV-infections was 45%, among these 17% with malignancies and 9% under immunosuppressive therapy. Rate of complications was not elevated in these two high-risk cohorts, but one ALL-patient died due to VZV-related complications. We report one 4-year old boy with initial diagnosis of acute lymphoblastic leukemia who showed a rapidly fatal outcome of his simultaneous varicella-infection, one 1.8-year old boy with an identical situation but a mild course of his disease, and an 8.5-year old boy with a steroiddependent nephrotic syndrome. This boy developed severe hepatic involvement during his varicella-infection but responded to immediate withdrawl of steroids and administration of acyclovir plus single-dose cidofovir after nonresponse to acyclovir after 48 h. Conclusion: Our data show that patients with malignant diseases or immunosuppressive therapy should be hospitalized and treated immediately with antiviral agents. Despite these measures the course of VZV-infections can be highly variable in these patients. We discuss aids to individual decision-making for these difficult situations.
Background and purpose
Pediatric adrenocortical carcinoma (pACC) is a rare disease with poor prognosis. Publications on radiotherapy (RT) are scarce. This review summarizes the current data on RT for pACC and possibly provides first evidence to justify its use in this setting.
Materials and methods
We searched the PubMed and Embase database for manuscripts regarding RT for pACC.
Results
We included 17 manuscripts reporting on 76 patients treated with RT, after screening 2961 references and 269 full articles. In addition, we added data of 4 unreported pACC patients treated by co-authors. All reports based on retrospective data. Median age at first diagnosis was 11.1 years (70% female); 78% of patients presented with hormonal activity. RT was mostly performed for curative intent (78%). 88% of RT were administered during primary therapy. The site of RT was predominantly the local tumor bed (76%). Doses of RT ranged from 15 to 62 Gy (median 50 Gy). Information on target volumes or fractionation were lacking. Median follow-up was 6,9 years and 64% of the patients died of disease, with 33% alive without disease. In 16 of 48 patients with available follow-up data after adjuvant RT (33%) no recurrence was reported and in 3 of 9 patients palliative RT seemed to induce some benefit for the patient.
Conclusions
Our first systematic review on RT for pACC provides too few data for any general recommendation, but adjuvant RT in patients with high risk might be considered. International collaborative studies are urgently needed to establish better evidence on the role of RT in this rare malignancy.
Einleitung: Eine nicht adäquate Funktion des Immunsystems ist ein großes klinisches Problem, welches Chemotherapien durch schwere bakterielle oder mykotische Infektionen komplikationsreich macht. Während einer Immunantwort spielen Zytokine, die von T-Zellen produziert werden, eine wichtige Rolle für die Effektivität der Antwort. Um zu verstehen, welche Veränderungen während der Therapie im Immunsystem auftreten, untersuchten wir die Subpopulationen und Funktionen (Zytokine , Immunglobuline) der Lymphozyten. Patienten: 23 Kinder (medianes Alter 5y; 2m-14y; 15 weiblich, 8 männlich) mit B-ALL behandelt nach dem ALL-BFM 2000-Protokoll. Blutproben wurden gesammelt bei Diagnosestellung und an den Tagen 8, 15, 33, 64 sowie vor Protokoll M, vor Protokoll 2 und während der Erhaltungstherapie. Methoden: Wir analysierten die Lymphozyten-Subpopulation mittels Durchflußzytometrie. Die Bestimmung intrazelluläre Zytokine (IFNγ, IL2, TNFα, IL4, IL5, and IL10) erfolgte durch FACS-Analysen nach in vitro Stimulation mit PMA, Ionomycin und Brefeldin für 24h. Zusätzlich untersuchten wir verschiedene Zytokine im unstimulierten Serum mittels ELISA und studierten TRECs und Spectratypes sowie die Immunglobulinlevels. Ergebnisse: Die B-Zellen verringerten sich schnell von einem Medianen Wert von 301+120/mm³ vor Chemotherapie auf 85+138/mm³ am Tag 33 und stiegen nicht mehr bis zum Ende der Therapie an. Die T-Zellzahl fiel zu Beginn der Therapie ab, allerdings konnten wir partielle Erholungen, die proportional zur Therapieintensität waren detektieren. Zudem fiel eine Verschiebung zugunsten der CD8+-Zellen auf. NK-Zellen zeigten keine signifikanten Veränderungen. Bei den von CD3+ -Zellen produzierten Zytokinen fiel eine Expressionssteigerung von IFNγ auf. Wir konnten eine Korrelation zwischen γδ-TCR und IFNγ -Produktion(FACS) sowie IFNγ -Werte(ELISA) und hohe Anzahl von Gedächtniszellen finden. Außerdem korrelieren CD45RA+Zellen mit CD4IL2+Zellen. TGFβ im unstimulierten Sera korrelierte signifikant (p<0,01) mit CD19+Zellen. TGFβ wurde auch von Blasten exprimiert. Wir stellten Unterschiede im Zytokinprofil zwischen dem mit Dexamethason bzw Prednison behandelten Patienten fest; insbesondere die IFNγ-Sekretion war sehr viel größer unter Prednisonbehandlung (p<0,01). TREC-Werte waren höher unter Dexamethason, aber das mag mit beeinflusst sein durch das Alter, da jüngere Kinder signifikant höhere TREC-Werte haben. Bei der Analyse des TZR-Repertoire zeigte sich eine höhere Komplexität im Prednisonzweig. Wir konnten Vβ-Genfamilien mit höherer Komplexität (BV22, BV23) und mit niedrigerer Komplexität (BV13b, BV6a) detektieren. Die Komplexität des TZR korrelierte positiv mit der Anzahl der naiven T-Zellen und dem Alter(p<0,01). Bis d15 nahm die Komplexität des Repertoires ab und erholte sich langsam im Therapieverlauf. Zusammenfassung: Wir detektierten eine Verschiebung zu Gunsten der TH1-Zytokine. B-Zellen wurden durch die Therapie ausgelöscht. Dieses Ergebnis mag eine klinische Relevanz haben in der prophylaktischen Gabe von Immunglobulinen, um schwere Infektionen durch das Fehlen der B-Zellen zu vermeiden.
We report on a nine-month-old girl who presented with persisting cough, and diminished ventilation of the left hemithorax. Viral pneumonia was suspected after Adenovirus detection by PCR, but chest X-rays showed a persistent shadowing of the left hemithorax and persistent coughing despite clinical improvement. Because of the discrepancy between clinical and radiological signs further investigations by ultrasound and CT scan were performed, which visualized an intrathroracic tumor. Histopathology confirmed diagnosis of a teratoma.
This case highlights the need for careful evaluation by the treating physicians. If the chest X-ray provides a discrepancy to the clinical findings or persistent pathologies exist, differential diagnosis should be discussed and further diagnostics be performed.
Background: The 2009 pandemic influenza A (H1N1) (PIA) virus infected large parts of the pediatric population with a wide clinical spectrum and an initially unknown complication rate. The aims of our study were to define clinical characteristics and outcome of pandemic influenza A (H1N1) 2009-associated hospitalizations (PIAH) in children <18 years of age. All hospitalized cases of children <18 years of age with laboratory-confirmed pandemic influenza A (H1N1) 2009 in the region of Wuerzburg (Northern Bavaria, Germany) between July 2009 and March 2010 were identified. For these children a medical chart review was performed to determine their clinical characteristics and complications. Results: Between July 2009 and March 2010, 94 PIAH (62% males) occurred in children <18 years of age, with a median age of 7 years (IQR: 3–12 years). Underlying diseases and predisposing factors were documented in 40 (43%) children; obesity (n = 12, 30%), asthma (n = 10, 25%) and neurologic disorders (n = 8, 20%) were most frequently reported. Sixteen (17%) children received oxygen supplementation; three (3%) children required mechanical ventilation. Six (6%) children were admitted to an intensive care unit, four of them with underlying chronic diseases. Conclusions: Most PIAH demonstrated a benign course of disease. However, six children (6%) needed treatment at an intensive care unit for severe complications.
Influenza gehört zu den häufigsten respiratorischen Erkrankungen im Kindesalter. Auf Grund der raschen weltweiten Verbreitung des im Frühjahr 2009 neu aufgetretenen pandemischen Influenza A (H1N1) 2009- Virus rief die Weltgesundheitsorganisation (WHO) am 11. Juni 2009 die höchste Pandemiestufe aus. Ziel der vorliegenden Studie war es, alle hospitalisierten Kinder < 18 Jahren im Raum Würzburg mit laborchemischem Nachweis von pandemischer Influenza A (H1N1) 2009 (PIA) zu erfassen und den klinischen Verlauf sowie die aufgetretenen Komplikationen zu beschreiben. Weiterhin wurde die PIA-assoziierte Hospitalisations-Inzidenz für das Haupteinzugsgebiet der Kliniken für verschiedene Altersgruppen berechnet und mit Hospitalisations-Inzidenzen für saisonale Influenza der Saisons 2006/2007 bis 2008/2009 verglichen.
Im Zeitraum von Juli 2009 bis März 2010 waren 94 Kinder < 18 Jahren (62% männlich) mit einer PIA-Infektion in stationärer Behandlung. Das Alter der Patienten lag im Median bei 7,1 Jahren (IQR: 3-12 Jahre). Eine Vorerkrankung bestand bei 40 Kindern (43%), wobei hier Adipositas (n=12, 30% der 40 Patienten mit Grunderkrankung), Asthma bronchiale (n=10, 25%) und neurologische Erkrankungen (n=8, 20%) am häufigsten waren. Die häufigsten Symptome bei stationärer Aufnahme waren Husten (n=75, 80% der 94 Patienten), Fieber (n=73, 78%) und Rhinitis (n=48, 51%). Eine Komplikation der PIA trat bei 37 Kindern (39%) auf, am häufigsten war eine Infektion der unteren Luftwege. Sechzehn Kinder (17%) benötigten eine Sauerstoffsupplementierung und drei Kinder (3%) wurden maschinell beatmet. Sechs Kinder (6%) wurden intensivstationär behandelt. Lediglich 3 Kinder (3%) waren gegen PIA und eines (1%) gegen saisonale Influenza geimpft. Die geschätzte Hospitalisations-Inzidenz im Würzburger Raum lag bei 118/100.000 für Kinder unter 18 Jahren und war am höchsten für Kinder unter einem Jahr (242/100.000). Die Hospitalisations-Inzidenz für saisonale Influenza A für Kinder unter 18 Jahren war in den drei vorhergehenden Saisons 2006/2007, 2007/2008 und 2008/2009 mit 41-97/100.000 niedriger als für PIA. Neben den 94 ambulant erworbenen Infektionen traten sieben nosokomiale Infektionen auf. Hierunter war ein Todesfall bei einem 16-jährigen Mädchen mit schweren Grunderkrankungen zu verzeichnen.
Trotz der hohen Hospitalisierungs-Inzidenz verlief der überwiegende Anteil an PIA-assoziierten Hospitalisierungen ohne schwere Komplikationen. Kinder mit einer Grunderkrankung waren häufiger von einer Komplikation der PIA betroffen und nur ein geringer Anteil dieser Patienten war gegen Influenza geimpft, obwohl eine explizite Impfempfehlung der Ständigen Impfkommission am Robert Koch-Institut für diese Patientengruppe besteht. Höhere Durchimpfungsraten sind notwendig, um die Krankheitslast und das Auftreten von Komplikationen zu reduzieren. Derzeit wird daher die Einführung einer generellen Empfehlung der Impfung gegen Influenza für Kinder diskutiert.
Perinatal inflammatory stress is strongly associated with adverse pulmonary outcomes after preterm birth. Antenatal infections are an essential perinatal stress factor and contribute to preterm delivery, induction of lung inflammation and injury, pre-disposing preterm infants to bronchopulmonary dysplasia. Considering the polymicrobial nature of antenatal infection, which was reported to result in diverse effects and outcomes in preterm lungs, the aim was to examine the consequences of sequential inflammatory stimuli on endogenous epithelial stem/progenitor cells and vascular maturation, which are crucial drivers of lung development. Therefore, a translational ovine model of antenatal infection/inflammation with consecutive exposures to chronic and acute stimuli was used. Ovine fetuses were exposed intra-amniotically to Ureaplasma parvum 42 days (chronic stimulus) and/or to lipopolysaccharide 2 or 7 days (acute stimulus) prior to preterm delivery at 125 days of gestation. Pulmonary inflammation, endogenous epithelial stem cell populations, vascular modulators and morphology were investigated in preterm lungs. Pre-exposure to UP attenuated neutrophil infiltration in 7d LPS-exposed lungs and prevented reduction of SOX-9 expression and increased SP-B expression, which could indicate protective responses induced by re-exposure. Sequential exposures did not markedly impact stem/progenitors of the proximal airways (P63+ basal cells) compared to single exposure to LPS. In contrast, the alveolar size was increased solely in the UP+7d LPS group. In line, the most pronounced reduction of AEC2 and proliferating cells (Ki67+) was detected in these sequentially UP + 7d LPS-exposed lambs. A similar sensitization effect of UP pre-exposure was reflected by the vessel density and expression of vascular markers VEGFR-2 and Ang-1 that were significantly reduced after UP exposure prior to 2d LPS, when compared to UP and LPS exposure alone. Strikingly, while morphological changes of alveoli and vessels were seen after sequential microbial exposure, improved lung function was observed in UP, 7d LPS, and UP+7d LPS-exposed lambs. In conclusion, although sequential exposures did not markedly further impact epithelial stem/progenitor cell populations, re-exposure to an inflammatory stimulus resulted in disturbed alveolarization and abnormal pulmonary vascular development. Whether these negative effects on lung development can be rescued by the potentially protective responses observed, should be examined at later time points.
