Einleitung
Fokale Gelenkknorpeldefekte treten in der Deutschen Bevölkerung mit einer geschätzten Inzidenz von über 300 000 jährlichen Fällen auf. In der US-amerikanischen Bevölkerung wird jährlich von über 600 000 Fällen ausgegangen. Aufgrund der Insuffizienz körpereigener Heilungskapazitäten und verfügbarer Therapieverfahren, schreitet die Erkrankung regelhaft zur post-traumatischen Arthrose fort. Neben der individuellen Lebensqualitätseinschränkung besteht eine sozioökonomische Bedeutung mit geschätzten Krankheitskosten von jährlich über 10 Milliarden US Dollar in den Vereinigten Staaten.
Das Versagen zellbasierter Therapieverfahren beruht unter anderem auf einer Insuffizienz der chondrogenen Differenzierung, sowie der hypertrophen Differenzierung der Chondrozyten mit nachfolgender Osteogenese analog den Vorgängen in der Wachstumsfuge. Für die Induktion der chondrogenen Differenzierung stehen insbesondere Mitglieder der TGF-β Superfamilie, wie BMP-2, zur Verfügung. Diese sind jedoch ebenso durch eine Induktion der hypertrophen Differenzierung gekennzeichnet. Zur Induktion der Chondrogenese unter Umgehung der TGF-β-Signalwege wurde IHH in-vitro als vielversprechend beschrieben. Bislang besteht jedoch kein Nachweis der in-vivo Effektivität von IHH zur Knorpelreparation.
Die Schaffung eines Wachstumsfaktor-Milieus in der Gelenkknorpelläsion in-vivo stellt ebenso eine Herausforderung dar. Diesbezüglich wurde ein vereinfachtes Verfahren zum lokalisierten in-vivo Gentransfer mittels adenoviraler Vektoren und autologen Knochenmarkskoagulaten anhand von Markergenen beschrieben. Die Effektivität jenes Verfahrens zur in-vivo Knorpelreparation wurde noch nicht gezeigt.
Zweck dieses kontrollierten in-vivo Experimentes ist es, mittels des oben genannten Gentransferverfahrens die Wirksamkeit von BMP-2 und IHH zur Reparation von osteochondralen Defekten in New Zealand White Rabbits nachzuweisen. Die zentrale Hypothese lautete, dass BMP2 beziehungsweise IHH Gentransfer in einer höheren langzeit-histologischen Qualität des Reparationsgewebes resultiert. Explorativ sollten dabei Unterschiede in den einzelnen Dimensionen der Gewebequalität anhand des ICRS-II Histology Scoring Systems, sowie der Grad der Typ I (als Faserknorpelmarker), Typ II (als Marker hyalinen Gelenkknorpels) und Typ X Kollagen Deposition (als Marker hypertropher Chondrozyten) beschrieben werden.
Material und Methoden
Als Tiermodel wurden bilaterale 3,2 mm durchmessende osteochondrale Bohrlochdefekte in der Trochlea von New Zealand White Rabbits verwendet (n=10 unabhängige Tiere, 20 Gelenke). Die Defekte wurden mit autologen Knochenmarkkoageln gefüllt, die nach vorheriger Beckenkammaspiration gewonnen wurden. In den experimentellen Gruppen wurden die Knochenmarkkoagel beladen mit jeweils 1 x 1011 infektiösen Partikeln adenoviraler Vektoren, die cDNA codierend für BMP2 (n=3 Tiere, entsprechend 6 Gelenken) oder IHH (n=4; 8) enthielten. In der Kontrollgruppe wurde das nicht-chondrogene Markergen GFP (n=3; 6) transferiert. Beide Gelenke eines Tieres wurden der gleichen Gruppe zugeordnet. Die histologische Gewebequalität wurde nach 13 Wochen anhand des ICRS-II Scoringsystems durch 3 unabhängige, verblindete Untersucher bewertet. Als primäre Outcomes wurden der ICRS-II Parameter „Generelles Assessment“, sowie die Typ II Kollagen positive Fläche designiert. Als explorative Outcomes wurden die verbleibenden ICRS-II Parameter, sowie die Typ I und Typ X Kollagen Deposition bewertet. Die Korrelation zwischen den Untersuchern wurde nach Pearson ermittelt. Zum Test auf Signifikanz der Gruppenunterschiede wurde ein lineares gemischtes Modell verwendet, welches einer mögliche Abhängigkeit beider Gelenke eines Tieres Rechnung trägt.
Ergebnisse
Qualitative Bewertung des Reparationsknorpels. Dreizehn Wochen nach der Intervention zeigten die meisten der BMP-2 behandelten Gelenke (4 von 6) und alle der IHH behandelten Gelenke (8 von 8) hyalin-artigen Reparationsknorpel, während alle GFP behandelten Kontrollgelenke (6 von 6) faserknorpel-artiges Reparationsgewebe zeigten. Zwei BMP-2 behandelten Gelenke zeigten eine ausgeprägte intraläsionale Knochenformation.
Primäre Outcomes - ICRS-II „Generelles Assessment“ und Typ II Kollagen positive Fläche. IHH und BMP-2 behandelte Gelenke zeigten im Vergleich zu GFP höhere Punktzahlen in dem ICRS-II „Generelles Assessment“ Parameter: +33.0 (95% Konfidenzintervall: -0.4, +66.4) Punkte für IHH und +8.5 (-26.6, +43.7) Punkte für BMP-2. Beide Effekte erreichten nicht das Level statistischer Signifikanz (p=0.052 und 0.537). IHH erhöhte die Typ II Kollagen Deposition in der Defektregion, während BMP-2 Gelenke keinen Unterschied zu GFP Kontrollen zeigten: +18.7 (-4.5, +42.0) Punkte für IHH und +0.0 (-29.7, +29.8) Punkte für BMP-2. Die erhöhte Typ II Kollagendeposition erreichte nicht das konventionelle Level statistischer Signifikanz (p=0.093).
Sekundäre Outcomes - ICRS-II Parameter. In dem Vergleich von BMP-2 mit GFP Kontrollen wurde in keinem der 12 untersuchten Parameter ein signifikanter Unterschied festgestellt. IHH Gentransfer resultierte hingegen in höheren Punktzahlen in allen untersuchten Parametern, wobei der Unterschied in 5 der 12 Parameter das Niveau statistischer Signifikanz erreichte. Ein um 21.5 Punkte (+3.6, +39.4) erhöhter Score wurde für den Parameter „Gewebemorphologie“ beobachtet, sowie +21.0 (+6.4, +35.7) für „Chondrozytäres Clustering“, +31.2 (+0.8, +61.5) für „Formation der Tidemark“, +17.3 (+0.2, +34.5) für „Abnorme Kalzifikation/Ossifikation“ und +35.0 (+4.6, +65.2) für das „Assessment der mittleren und tiefen Zone“.
Sekundäre Outcomes - Marker chondrozytärer Hypertrophie. Eine perizelluläre Deposition von Typ X Kollagen wurde in allen Gruppen beobachtet. Eine deutlich gesteigerte Deposition wurde nur in den Gelenken beobachtet, die nach BMP2 Gentransfer eine ausgeprägte intraläsionale Knochenformation zeigten.
Diskussion
Das hier beschriebene Experiment stellt die erste Veröffentlichung der Wirksamkeit von IHH zur Verbesserung der histologischen Knorpelqualität von in-vivo therapierten Gelenkknorpeldefekten dar [175]. Die Hypothese, dass IHH zu einer verbesserten histologischen Knorpelqualität führt wurde bestätigt, während die Hypothese zu den positiven Effekten von BMP-2 wiederlegt wurde. IHH führte zu besseren Ergebnissen in allen Untersuchten Parametern, das Niveau statistischer Signifikanz wurde dabei in den Parametern „Gewebemorphologie“, „Chondrozytäres Clustering“, „Formation der Tidemark“, „Abnorme Kalzifikation/Ossifikation“ und „Assessment der mittleren und tiefen Zone“ erreicht. Das primäre Ziel dieses Experimentes war es, den „Proof of concept“ zu liefern, dass IHH auch in-vivo ein attraktiver Faktor für die Induktion der Chondrogenese darstellt. Das langfristige Ziel ist die Induktion der Chondrogenese unter Umgehung des TGF-β Signalweges zu erzielen, um eine folgende hypertrophe Differenzierung der Chondrozyten und die folgende Ossifikation des reparierten Defektes zu verhindern.
Die Limitationen der Studie umfassen die ausschließlich histologische und immunhistochemische durchgeführte Bewertung der Knorpelqualität und eine eingeschränkte statistische Power. Ob IHH es vermag die hypertrophe Differenzierung zu umgehen und somit eine langfristige hyaline Knorpelreparation zu ermöglichen, ist in weiteren präklinischen Studien mit biochemischer und molekulargenetischer Analyse der Hypertrophie-Marker zu untersuchen. In Bezug auf den klinischen Einsatz zur Knorpelreparation erscheint der Einsatz der Wachstumsfaktoren als Protein auf funktionalisierten Matrices vielversprechend.
BMP-2 wird aufgrund der hier beobachteten intraläsionalen Knochenformation nach BMP2 Gentransfer als nicht geeignet zur Unterstützung der Knorpelreparation in-vivo bewertet.
Das System Thioredoxin /Thioredoxin Reduktase(Trx/TrxR) ist ein sehr versatiles System zur neutralisation reaktiver Sauerstoffspezies, zur Regulation redox-sensitiver Vorgänge und zur Aktivierung von Transkriptionsfaktoren wie Steroidhormonrezeptoren, AP-1 und NFkB. Das Enzym Thioredoxin Reduktase war zunächst nur als zytosolisches Enzym beschrieben, es ist mittlerweile bekannt, dass es z. B. nach Phorbolester-Stimulation auch sezeniert werden kann. Adäquate Stimuli für die nucläere Translokation von Trx sind z. B. UV-Licht und TNF-Signalling. Zudem wurde in der vorhandenen Arbeit anhand transienter Transfektion und immunhistochemischer Untersuchungen nachgewiesen, dass beide Komponenten des Systems auch im Zellkern präsent sind. Ein Teil er Arbeit stellt die Charakteriesierung der subzellulären Lokalisation zweier Isoformen von Thioredoxin Reduktase 1 mit unterschiedlichem N-Terminus dar. Es konnte gezeigt werden, dass die beiden Isoformen als mRNA und Protein vorhanden sind. Es wurde dann die Interaktion des Enzyms Thioredoxin Reduktase mit anderen Komponenten des Zellkerns, hier speziell mit Enzymen der DNA-Prozessierung untersucht. Zudem wurde in einem Immunpräzipitationsansatz ("Pull-Down-Assay") nucläere Interaktionspartner des Enzyms charakterisiert. Diese Partner sollen nach Gelelektrophorese und MALDI-TOF-Analyse identifiziert werden. Zu den DNA-Prozessierungsenzyme zählt auch Topisomerase I. Durch Antikörpervermittelte Assays gelang es nachzuweisen, dass Topoisomerase I mit TrxR eine Protein-Protein-Wechsekwirkung eingeht. In einem Rekonstruktionssystem mit rekombinanter Topoisomerase I und gerenigter TrxR ergab sich jedoch keiner Hinweis für eine funktionelle Interaktion in DNA-Relaxations-Assay. Die Aufschlüsselung der Protein-Protein-Interaktion, der detaillierten molekularen Mechanismen und ihrer physiologischen relevanz bleibt weiteren Unterschungen vorbehalten.
Introduction
Diagnosis and treatment of insertional tendinopathy of the Achilles tendon (IAT) remains a challenge. The aim of this study was to assess the influence of pre-operative radiological pathologies on the patient-reported outcomes following open debridement of all pathologies for IAT.
Materials and methods
In this IRB-approved retrospective correlation and comparative study, patients with pre-operative imaging were identified from the authors’ retrospective IAT database comprising of 118 patients. All were treated by a standardized surgical treatment strategy utilizing a midline, transachillary approach and debridement of all pathologies. A total of fifteen radiologic parameters were measured on radiographs (RX) and MRI. The patient-reported outcomes were assessed using the Victorian Institute of Sport Assessment-Achilles questionnaire (VISA-A-G) and the general health questionnaire SF-12 at a minimum follow-up of 12 months. The data are presented as mean ± SD (95% CI).
Results
88 patients (74.6%) with an average age of 50 ± 12 (47–52) years were included. Radiographs were available in 68 patients and MRI in 53. The mean follow-up was 3.8 ± 1.9 (3.4–4.3) years. The overall VISA-A-G was 81 ± 22 (77–86), the SF-12 PCS 54 ± 7 (52–55), and the SF-12 MCS 52 ± 9 (50–54) points. None of the assessed radiological parameters had a significant influence on the patient-reported outcome following surgical treatment for IAT.
Conclusion
In this retrospective correlation study, no significant association was found between preoperative radiographic and MRI radiologic parameters for IAT and postoperative patient-reported outcomes (VISA-A-G and SF-12).
Vitamin D deficiency is a global health concern that is estimated to afflict over one billion people globally. The major role of vitamin D is that of a regulator of calcium and phosphate metabolism, thus, being essential for proper bone mineralisation. Concomitantly, vitamin D is known to exert numerous extra-skeletal actions. For example, it has become evident that vitamin D has direct anti-proliferative, pro-differentiation and pro-apoptotic actions on cancer cells. Hence, vitamin D deficiency has been associated with increased cancer risk and worse prognosis in several malignancies. We have recently demonstrated that vitamin D deficiency promotes secondary cancer growth in bone. These findings were partly attributable to an increase in bone remodelling but also through direct effects of vitamin D on cancer cells. To date, very little is known about vitamin D status of patients with bone tumours in general. Thus, the objective of this study was to assess vitamin D status of patients with diverse bone tumours. Moreover, the aim was to elucidate whether or not there is an association between pre-diagnostic vitamin D status and tumour malignancy in patients with bone tumours.
In a multi-center analysis, 25(OH)D, PTH and calcium levels of 225 patients that presented with various bone tumours between 2017 and 2018 were assessed. Collectively, 76% of all patients had insufficient vitamin D levels with a total mean 25(OH)D level of 21.43 ng/ml (53.58 nmol/L). In particular, 52% (117/225) of patients were identified as vitamin D deficient and further 24% of patients (55/225) were vitamin D insufficient. Notably, patients diagnosed with malignant bone tumours had significantly lower 25(OH)D levels than patients diagnosed with benign bone tumours [19.3 vs. 22.75 ng/ml (48.25 vs. 56.86 nmol/L); p = 0.04).
In conclusion, we found a widespread and distressing rate of vitamin D deficiency and insufficiency in patients with bone tumours. However, especially for patients with bone tumours sufficient vitamin D levels seem to be of great importance. Thus, we believe that 25(OH)D status should routinely be monitored in these patients. Collectively, there should be an increased awareness for physicians to assess and if necessary correct vitamin D status of patients with bone tumours in general or of those at great risk of developing bone tumours.
48 Patienten mit Rotatorenmanschettenrupturen (39-mal SSP, 3-mal SSP+ISP, 2-mal SSP+SSC, 4-mal SSP+ISP+SSC) wurden präoperativ sowie postoperativ nach sechs und sieben Wochen klinisch und sonographisch untersucht. Die klinische Untersuchung umfasste die Bestimmung des Constant-Scores sowie die Bewertung der dynamischen Schulterblattbewegung. Die sonographische Untersuchung erfolgte nach der von Müller et al. (1998) beschriebenen Methode, die eine standardisierte Dickenmessung der Mm. SSP und ISP, die Beurteilung ihrer Echogenitäten und Abgrenzbarkeiten sowie die Bewertung ihrer Kinetik bei definierten Bewegungen der Arme beinhaltete. Präoperativ lagen bei 40 Patienten pathologische Schulterblattbewegungsmuster vor. Mit Beginn der aktiven Rehabilitation kam es bei der Armbewegung in Abduktionsrichtung zu einer im Mittel signifikanten Besserung. Die Echogenität von M. SSP und M. ISP war bei intakter Sehne normal, bei Defekten präoperativ oft verändert und zeigte postoperativ eine weitere Verschlechterung. Der M. SSP war bei den Männern prä-, postoperativ und unabhängig davon, ob seine Sehne gerissen war, signifikant dicker als bei den Frauen. Für den M. ISP galt dies nur bei intakter Sehne. Die postoperative Ruhigstellung führte zu einer signifikanten Atrophie des M. ISP mit intakter Sehne. Die Mm. SSP und ISP mit Sehnenruptur waren schon präoperativ signifikant schmäler als die der gesunden Gegenseite und veränderten sich im postoperativen Verlauf nicht mehr. Die postoperative Ruhigstellung führte zu einer signifikanten Verschlechterung der ursprünglich normalen Muskelkinetik des M. ISP ohne Sehnenruptur. Bei den Mm. SSP und ISP mit Sehnenruptur war die Kinetik schon präoperativ signifikant schlechter als auf der Gegenseite und verschlechterte sich im postoperativen Verlauf noch weiter. Muskeln mit postoperativ rerupturierten Sehnen zeigten eine schlechterte Muskelkinetik als jene mit erfolgreich operierten Sehnen. Sowohl prä- als auch postoperativ korrelierten Echogenität, Abgrenzbarkeit und Muskelkinetik sehr gut mit der Defektgröße und dem postoperativen Constant-Score. Hingegen war die Korrelation der dynamischen Schulterblattbewegung mit Defektgröße und Constant-Score nur sehr schwach. Die verwendete Methode der Bestimmung der dynamischen Schulterblattbewegung ist zur Verlaufsbeobachtung einer operierten Rotatorenmanschette, zur Prognose über die tatsächliche Defektgröße und das funktionelle Ergebnis nach sieben Wochen nur begrenzt geeignet. Die sonographische Methode hingegen eignet sich als zusätzliches diagnostisches Kriterium bei Defekten der Rotatorenmanschette, hilft bei der Abschätzung der Defektgröße, ermöglicht eine prognostische Aussage über die postoperativ zu erwartende Funktion und bietet die Möglichkeit der objektiven Befundkontrolle während der Rehabilitation.
Es sollte eine dynamische MRT Methode entwickelt werden, welche die Berechnung und Darstellung einer Achse einer komplexen Bewegung ermöglicht. Dabei war die eigens auferlegte Definition von „Dynamik“, dass ein kompletter Bewegungsablauf als dreidimensionaler Datensatz im Zeitverlauf erfasst und ausgewertet werden soll (4 Dimensionen).
Diese Methode sollte auf die komplexe Bewegung des Kniegelenks angewendet werden und mit einem Knie-Modell verglichen werden.
Schlussendlich sollte die Flexion-Extensionsbewegung als vereinfacht monoaxial verifiziert werden.
Für diese Ziele wurden folgende Schritte durchgeführt:
Es wurden zunächst 10 Kadaver-Knie präpariert und hierbei u.a. kontrastmittelhaltige Kugeln als Tracker eingebracht. Die Knie wurden dann im Rahmen des Versuchsaufbaus in einer speziellen, pneumatischen Bewegungsschiene befestigt, welche dann automatisiert eine passive Bewegung vollzog. Diese wurde mit einer dynamischen MRT Methode festgehalten. Der entstandene 4D-Datensatz wurde eingelesen und die Kugelpositionen ausgewertet. Die Koordinaten der Positionen dienten dann als Grundlage für spezielle mathematische Algorithmen, welche die Flexion-Extensionsachse und ggf. die Innen- und Außenrotationsachse berechneten und schließlich eine graphische Darstellung ermöglichten. Zur Überprüfung wurde zudem ein Knie-Modell herangezogen und von diesem die gleichen Berechnungen durchgeführt.
Background: To describe changes in health-related quality of life (HRQoL) of postmenopausal women with osteoporosis treated with teriparatide for up to 18 months and followed-up for a further 18 months, and to assess the influence of recent prior and incident fractures.
Methods: The European Forsteo Observational Study (EFOS) is an observational, prospective, multinational study measuring HRQoL using the EQ-5D. The primary objective was to assess changes in HRQoL during 36 months in the whole study population. A secondary post-hoc analysis examined fracture impact on HRQoL in four subgroups classified based on recent prior fracture 12 months before baseline and incident clinical fractures during the study. Changes from baseline were analysed using a repeated measures model.
Results: Of the 1581 patients, 48.4% had a recent prior fracture and 15.6% of these patients had an incident fracture during follow-up. 10.9% of the 816 patients with no recent prior fracture had an incident fracture. Baseline mean EQ-VAS scores were similar across the subgroups. In the total study cohort (n = 1581), HRQoL (EQ-VAS and EQ-5D index scores) improved significantly from baseline to 18 months and this improvement was maintained over the 18-month post-teriparatide period. Improvements were seen across all five EQ-5D domains during teriparatide treatment that were maintained after teriparatide was discontinued. Subjects with incident clinical fractures had significantly less improvement in EQ-VAS than those without incident fractures. Recent prior fracture did not influence the change in EQ-VAS during treatment.
Conclusions: EFOS is the first longitudinal study in women with severe postmenopausal osteoporosis in the real world setting to show a substantial improvement in HRQoL during teriparatide treatment that was sustained during subsequent treatment with other medications. The increase in HRQoL was lower in the subgroups with incident fracture but was not influenced by recent prior fracture. The results should be interpreted in the context of the design of an observational study.
This predefined analysis of the European Forsteo Observational Study (EFOS) aimed to describe clinical fracture incidence, back pain, and health-related quality of life (HRQoL) during 18 months of teriparatide treatment and 18 months post-teriparatide in the subgroup of 589 postmenopausal women with osteoporosis aged ≥75 years. Data on clinical fractures, back pain (visual analogue scale, VAS), and HRQoL (EQ-5D) were collected over 36 months. Fracture data were summarized in 6-month intervals and analyzed using logistic regression with repeated measures. A repeated-measures model analyzed changes from baseline in back pain VAS and EQ-VAS. During the 36-month observation period, 87 (14.8 %) women aged ≥75 years sustained a total of 111 new fractures: 37 (33.3 %) vertebral fractures and 74 (66.7 %) nonvertebral fractures. Adjusted odds of fracture was decreased by 80 % in the 30 to <36–month interval compared with the first 6-month interval (P < 0.009). Although the older subgroup had higher back pain scores and poorer HRQoL at baseline than the younger subgroup, both age groups showed significant reductions in back pain and improvements in HRQoL postbaseline. In conclusion, women aged ≥75 years with severe postmenopausal osteoporosis treated with teriparatide in normal clinical practice showed a reduced clinical fracture incidence by 30 months compared with baseline. An improvement in HRQoL and, possibly, an early and significant reduction in back pain were also observed, which lasted for at least 18 months after teriparatide discontinuation when patients were taking other osteoporosis medication. The results should be interpreted in the context of an uncontrolled observational study.
In the adult skeleton, bone remodeling is required to replace damaged bone and functionally adapt bone mass and structure according to the mechanical requirements. It is regulated by multiple endocrine and paracrine factors, including hormones and growth factors, which interact in a coordinated manner. Because the response of bone to mechanical signals is dependent on functional estrogen receptor (ER) and Wnt/β-catenin signaling and is impaired in postmenopausal osteoporosis by estrogen deficiency, it is of paramount importance to elucidate the underlying mechanisms as a basis for the development of new strategies in the treatment of osteoporosis. The present study aimed to investigate the effectiveness of the activation of the ligand-dependent ER and the Wnt/β-catenin signal transduction pathways on mechanically induced bone formation using ovariectomized mice as a model of postmenopausal bone loss. We demonstrated that both pathways interact in the regulation of bone mass adaption in response to mechanical loading and that the activation of Wnt/β-catenin signaling considerably increased mechanically induced bone formation, whereas the effects of estrogen treatment strictly depended on the estrogen status in the mice.
Osteoporosis, or steroid-induced osteonecrosis of the hip, is accompanied by increased bone marrow adipogenesis. Such a disorder of adipogenic/osteogenic differentiation, affecting bone-marrow-derived mesenchymal stem cells (BMSCs), contributes to bone loss during aging. Here, we investigated the effects of extracellular vesicles (EVs) isolated from human (h)BMSCs during different stages of osteogenic differentiation on the osteogenic and adipogenic differentiation capacity of naïve (undifferentiated) hBMSCs. We observed that all EV groups increased viability and proliferation capacity and suppressed the apoptosis of naïve hBMSCs. In particular, EVs derived from hBMSCs at late-stage osteogenic differentiation promoted the osteogenic potential of naïve hBMSCs more effectively than EVs derived from naïve hBMSCs (naïve EVs), as indicated by the increased gene expression of COL1A1 and OPN. In contrast, the adipogenic differentiation capacity of naïve hBMSCs was inhibited by treatment with EVs from osteogenic differentiated hBMSCs. Proteomic analysis revealed that osteogenic EVs and naïve EVs contained distinct protein profiles, with pro-osteogenic and anti-adipogenic proteins encapsulated in osteogenic EVs. We speculate that osteogenic EVs could serve as an intercellular communication system between bone- and bone-marrow adipose tissue, for transporting osteogenic factors and thus favoring pro-osteogenic processes. Our data may support the theory of an endocrine circuit with the skeleton functioning as a ductless gland.
The present study aimed to evaluate the effect of high intensity dynamic resistance exercise (HIT-DRT) and whey protein supplementation (WPS) on bone mineral density (BMD) and sarcopenia parameters in osteosarcopenic men. Men ≥ 72 years with osteosarcopenia (n = 43) were randomly assigned to a HIT-RT (HIT-RT: n = 21) or a non-training control group (n = 22). Supervised HIT-RT twice/week was applied for 18 months, while the control group maintained their habitual lifestyle. Supplying WPS, total protein intake amounted to 1.5–1.6 (HIT-RT) and 1.2 g/kg/body mass/d (control). Both groups were supplied with calcium and vitamin D. Primary study outcomes were BMD and the sarcopenia Z-score. After adjusting for multiplicity, we observed significant positive effects for sarcopenia Z-score (standardized mean difference (SMD): 1.40), BMD at lumbar spine (SMD: 0.72) and total hip (SMD: 0.72). In detail, effect sizes for skeletal muscle mass changes were very pronounced (1.97, p < 0.001), while effects for functional sarcopenia parameters were moderate (0.87, p = 0.008; handgrip strength) or low (0.39, p = 0.209; gait velocity). Apart from one man who reported short periods of temporary worsening of existing joint pain, no HIT-RT/WPS-related adverse effects or injuries were reported. We consider HIT-RT supported by whey protein supplementation as a feasible, attractive, safe and highly effective option to fight osteosarcopenia in older men.
Dynamic resistance exercise (DRT) might be the most promising agent for fighting sarcopenia in older people. However, the positive effect of DRT on osteopenia/osteoporosis in men has still to be confirmed. To evaluate the effect of low‐volume/high‐intensity (HIT)‐DRT on bone mineral density (BMD) and skeletal muscle mass index (SMI) in men with osteosarcopenia, we initiated the Franconian Osteopenia and Sarcopenia Trial (FrOST). Forty‐three sedentary community‐dwelling older men (aged 73 to 91 years) with osteopenia/osteoporosis and SMI‐based sarcopenia were randomly assigned to a HIT‐RT exercise group (EG; n = 21) or a control group (CG; n = 22). HIT‐RT provided a progressive, periodized single‐set DRT on machines with high intensity, effort, and velocity twice a week, while CG maintained their lifestyle. Both groups were adequately supplemented with whey protein, vitamin D, and calcium. Primary study endpoint was integral lumbar spine (LS) BMD as determined by quantitative computed tomography. Core secondary study endpoint was SMI as determined by dual‐energy X‐ray absorptiometry. Additional study endpoints were BMD at the total hip and maximum isokinetic hip−/leg‐extensor strength (leg press). After 12 months of exercise, LS‐BMD was maintained in the EG and decreased significantly in the CG, resulting in significant between‐group differences (p < 0.001; standardized mean difference [SMD] = 0.90). In parallel, SMI increased significantly in the EG and decreased significantly in the CG (p < 0.001; SMD = 1.95). Total hip BMD changes did not differ significantly between the groups (p = 0.064; SMD = 0.65), whereas changes in maximum hip−/leg‐extensor strength were much more prominent (p < 0.001; SMD = 1.92) in the EG. Considering dropout (n = 2), attendance rate (95%), and unintended side effects/injuries (n = 0), we believe our HIT‐RT protocol to be feasible, attractive, and safe. In summary, we conclude that our combined low‐threshold HIT‐RT/protein/vitamin D/calcium intervention was feasible, safe, and effective for tackling sarcopenia and osteopenia/osteoporosis in older men with osteosarcopenia.
Testosterone deficiency in males is linked to various pathological conditions, including muscle and bone loss. This study evaluated the potential of different training modalities to counteract these losses in hypogonadal male rats. A total of 54 male Wistar rats underwent either castration (ORX, n = 18) or sham castration (n = 18), with 18 castrated rats engaging in uphill, level, or downhill interval treadmill training. Analyses were conducted at 4, 8, and 12 weeks postsurgery. Muscle force of the soleus muscle, muscle tissue samples, and bone characteristics were analyzed. No significant differences were observed in cortical bone characteristics. Castrated rats experienced decreased trabecular bone mineral density compared to sham-operated rats. However, 12 weeks of training increased trabecular bone mineral density, with no significant differences among groups. Muscle force measurements revealed decreased tetanic force in castrated rats at week 12, while uphill and downhill interval training restored force to sham group levels and led to muscle hypertrophy compared to ORX animals. Linear regression analyses showed a positive correlation between bone biomechanical characteristics and muscle force. The findings suggest that running exercise can prevent bone loss in osteoporosis, with similar bone restoration effects observed across different training modalities.
In einer randomisierten und doppelblinden Studie wurde der Effekt der frühen postoperativen Akupunktur auf die passive Beugefähigkeit im Kniegelenk und Schmerzreduktion nach Knieprothesenimplantation untersucht. Der Untersuchungszeitraum lag zwischen dem 2. und 6. postoperativen Tag. In der Auswertung der Studie zeigte sich eine Verbesserung der Beugefähigkeit unter Verringerung der Ruheschmerzintensität in der Verum-Akupunkturgruppe. Die statistische Auswertung ergab hier jedoch keine ausreichenden signifikanten Unterschiede zwischen den Vergleichsgruppen.
Hintergrund: Heterotope Ossifikation (HO) stellt, mit einer Inzidenz von bis zu 90 %, eine häufige Komplikation nach Hüfttotalendoprothetik dar. Für höhergradige HO mit klinischer Relevanz (schwere HO) wird eine Inzidenz von bis zu 20 % berichtet. Durch eine Prophylaxe, mit nichtsteroidalen Antirheumatika (NSAID) oder Strahlentherapie, kann die Inzidenz für schwere HO auf 1 bis 5 % gesenkt werden. Aus ökonomischen und logistischen Gesichtspunkten erfolgt bevorzugt eine Prophylaxe mit NSAID. Es liegen jedoch nur wenige Informationen zur Auswahl des Wirkstoffes, der Therapiedauer und der Dosierung vor.
Methoden: Ziel war die Bewertung der Wirksamkeit und Sicherheit verschiedener Interventionen mit NSAID, für die primären Endpunkte schwere HO und unerwünschte Arzneimittelwirkung (UAW). Hierzu war eine systematische Übersichtsarbeit mit Netzwerk-Metaanalyse geplant.
Es wurden 31 Studien mit 5128 Patienten eingeschlossen. Hieraus wurde ein Netzwerk aus 15 Knotenpunkten mit 44 Vergleichen erstellt. Die Studien wiesen eine ausgeprägte Heterogenität auf. Das Biasrisiko wurde häufig als unklar oder hoch bewertet. Eine Netzwerk-Metaanalyse konnte aufgrund der festgestellten Inkohärenz mit schwachen Verbindungen nicht durchgeführt werden. Es erfolgten ausschließlich Analysen der direkten Vergleiche.
Ergebnisse: Die Analysen zeigten, dass bereits eine kurze Interventionsdauer (bis 7 Tage) mit geringer Tagesdosis einen signifikanten Effekt zur Prophylaxe vor schwerer HO, bei nicht erhöhtem Risiko für schwere UAW aufweist. Ein überlegener Wirkstoff konnte nicht identifiziert werden.
Zusammenfassung: Die Evidenz dieser Dissertation wird als gering eingestuft, da aufgrund der spezifischeren Fragestellung als in den eingeschlossenen Studien, eine unzureichende Power bei den Analysen vorlag. Weitere qualitativ hochwertige RCT mit klinisch orientierter Fragestellung sind erforderlich, um die Frage nach einer optimalen Prophylaxe vor schwerer HO zu beantworten.
Bone marrow lesions (BML) represent areas of deteriorated bone structure and metabolism characterized by pronounced water‐equivalent signaling within the trabecular bone on magnetic resonance imaging (MRI). BML are associated with repair mechanisms subsequent to various clinical conditions associated with inflammatory and non‐inflammatory injury to the bone. There is no approved treatment for this condition. Bisphosphonates are known to improve bone stability in osteoporosis and other bone disorders and have been used off‐label to treat BML. A randomized, triple‐blind, placebo‐controlled phase III trial was conducted to assess efficacy and safety of single‐dose zoledronic acid (ZOL) 5 mg iv with vitamin D 1000 IU/d as opposed to placebo with vitamin D 1000 IU/d in 48 patients (randomized 2:1) with BML. Primary efficacy endpoint was reduction of edema volume 6 weeks after treatment as assessed by MRI. After treatment, mean BML volume decreased by 64.53% (±41.92%) in patients receiving zoledronic acid and increased by 14.43% (±150.46%) in the placebo group (p = 0.007). A decrease in BML volume was observed in 76.5% of patients receiving ZOL and in 50% of the patients receiving placebo. Pain level (visual analogue scale [VAS]) and all categories of the pain disability index (PDI) improved with ZOL versus placebo after 6 weeks but reconciled after 6 additional weeks of follow‐up. Six serious adverse events occurred in 5 patients, none of which were classified as related to the study drug. No cases of osteonecrosis or fractures occurred. Therefore, single‐dose zoledronic acid 5 mg iv together with vitamin D may enhance resolution of bone marrow lesions over 6 weeks along with reduction of pain compared with vitamin D supplementation only.
Der MPFL-Ersatz ist ein gängiges Therapieverfahren zur Behandlung einer patellofemoralen Instabilität. Die Operationstechniken unterscheiden sich zumeist an der patellaren Fixationsmethode und der Auswahl der Transplantate. Biomechanische Studien, welche sich mit den Eigenschaften implantatfreier ossärer Fixationsmethoden beim MPFL-Ersatz unter Verwendung künstlicher Transplantate beschäftigen gibt es nach aktueller Recherche nicht. Ziel dieser Arbeit war es, die biomechanischen Eigenschaften zweier patellarer Bohrkanalfixationstechniken beim MPFL-Ersatz mit unterschiedlichem künstlichen Bandersatzmaterial zu ermitteln. Die Hypothese war, dass die biomechanischen Eigenschaften in Elongation, Steifigkeit, Primärstabilität und maximaler Ausreißkraft mit denen der bereits etablierten Verfahren und dem nativen MPFL vergleichbar sind. Hierzu wurden 80 porcine Kniescheiben randomisiert in 8 Gruppen aufgeteilt und getestet. In den Gruppen 1-4 wurden parallele, transpatellare Bohrkanäle mit Tapes der Breiten 2 mm, 3 mm, 4 mm und 5 mm getestet. In den Gruppen 5-8 wurden V-Kanal-Fixationsmethoden mit Bändern der Breite von 2 mm, 3 mm, 4 mm und 5 mm untersucht. Zusätzlich wurden die biomechanischen Grundeigenschaften der nativen Tapes ermittelt. Alle Tests durchliefen jeweils drei Messabschnitte. Hierbei fand zunächst eine Präkonditionierung mit 10 Zyklen zwischen 5 N und 20 N statt. Daraufhin folgte eine zyklische Belastung mit 1000 Zyklen zwischen 5 N und 50 N. Am Ende wurde eine maximale Kraftapplikation bis zum Versagen der Fixationskomplexe durchgeführt. Im Rahmen der Messungen wurden Elongation, Steifigkeit, Yield Load und Maximum Load bestimmt. Es konnten Unterschiede zwischen den beiden Fixationsmethoden und den verwendeten Tapes festgestellt werden. Alle acht Gruppen zeigten eine höhere Primärstabilität als das humane MPFL. Bezogen auf die biomechanischen Eigenschaften und den Versagensmechanismus konnte in dieser Studie ein Vorteil der parallelen transpatellaren Bohrkanäle gegenüber den V- Kanaltechniken festgestellt werden. Die Werte mit der höchsten maximalen Ausreißkraft wurden in Gruppe 3 (631,6 ± 83,1 N) und Gruppe 1 (592,9 ± 170,1 N) gemessen. Diese zeigten eine höhere Primärstabilität mit geringerer Elongation und Steifigkeit im Vergleich zu den in der aktuellen Literatur beschriebenen biomechanischen Studien, welche sich mit unterschiedlichen und teilweise bereits etablierten MPFL-Ersatzverfahren beschäftigten. Eine implantatfreie MPFL-Rekonstruktion mit transpatellaren parallelen Bohrkanälen unter Verwendung eines 2 mm Fiber Tapes (Fa. Arthrex) oder eines 4 mm Tapes (Fa. Topester) könnten dementsprechend eine gute Alternative zur operativen Therapie einer patellofemoralen Instabilität sein.

10-jahres Ergebnisse einer prospektiven, randomisierten klinischen Studie.

Diese Studie vergleicht die Ergebnisse einer 10-Jahres Nachuntersuchung nach arthroskopisch unterstütztem Ersatz des vorderen Kreuzbandes mittels zweier verschiedener autologer Transplantate. Zwischen November 1998 und November 1999 wurden in der Orthopädischen Universitätsklinik König-Ludwig-Haus in Würzburg 62 Studienteilnehmer (45 Männer, 17 Frauen) mit Ruptur des vorderen Kreuzbandes operiert. Bei 31 Patienten erfolgte die ACL-Ersatzplastik mittels Lig. Patellae Transplantat (BTB-Transplantat), bei der anderen Hälfte mittels Semitendinosussehnentransplantat. Die Gruppenzusammensetzung zeigte vor dem Verletzungszeitpunkt keinen signifikanten Unterschied bezüglich Alter, Geschlecht und Aktivitätsniveau. Alle Patienten erhielten die gleiche postoperative Behandlung mit Don-Joy Schiene (Ext/Flex 0/0/90), Teilbelastung bis 20 kg während der ersten 6 Wochen und externe Rehabilitationsmaßnahmen. Nach über 10 Jahren konnten 47 (24 LP/23 ST) Patienten vollständig klinisch untersucht und bei 9 (5 LP/4 ST) weiteren zumindest standardisierte Fragebögen ausgewertet werden. 2 Studienteilnehmer (beide ST) schieden wegen einer Transplantatruptur, ein weiterer Patient (ebenfalls ST) aufgrund einer postoperativen Kniegelenksinfektion vorzeitig aus der Studie aus.
Ergebnisse:
Die klinische Untersuchung ergab bezüglich der verwendeten Parameter IKDC- und Lysholm-Score, Tegener-Aktivitätsskala, KT-1000-Arthrometer-Messung und radiologischer Kontrolle keine signifikanten Unterschiede zwischen den beiden Gruppen. Auch in den verwendeten Fragebögen mit Evaluation der subjektiven Kniefunktion, möglicher Schmerzen im Kniegelenk, Entnahmestellenmorbidität und der Zufriedenheit mit dem Operationsergebnis konnten keine signifikanten Gruppenunterschiede festgestellt werden.
In der vorliegenden Studie fanden sich somit auch über 10 Jahre nach vorderem Kreuzbandersatz keine relevanten Unterschiede oder Vorteile für eines der beiden untersuchten autologen Sehnentransplantate.
Hintergrund: Das Ziel der Studie war es, die Resultate nach arthroskopischer Rekonstruktion des vorderen Kreuzbandes (ACL)unter Verwendung der zentral-dritten Knochen-Patellarsehne-Knochen (LP) Autografts und der dreifachen/vierfachen Semitendinosus (ST) Autografts zu vergleichen. Hypothese: In der untersuchten Zeitdauer bietet die ACL-Rekonstruktion unter Verwendung der LP-Autografts mehr Stabilität aber zeigt auch mehr Entnahmestellenprobleme als ST-Autografts. Studienentwurf: Randomisierte kontrollierte Studie; Level of Study 1. Methoden: Eine randomisierte Reihe von 62 Patienten (17 Frauen und 45 Männer) mit einer einseitigen ACL-Ruptur, die rekonstrutive Chirurgie durchmachten, waren in der Studie eingeschlossen. Die LP-Transplantation (LP Gruppe) und die Semitensinosussehne Transplantation (ST-Gruppe) wurden gleichmäßig bei 31 Patienten durchgeführt. Die Patienten wurden in einer mittleren Zeitdauer von 67 Monaten (Range 62 bis 84 Monate) nach der Rekonstruktion nachuntersucht. Resultate: 44 von 62 Patienten (71%) konnten am Anschluss nachuntersucht werden. Die klinischen Untersuchungen zeigten keine signikanten Unterschiede zwischen der LP-Gruppe und der ST-Gruppe in der Lysholm-Score, Tegner-Aktivitätsniveau, IKDC-auswertungssystem, Einbeinsprungtest, KT-1000-Arthrometer, manueller Lachmantest auf. Eine bedeutende klinische Verbesserung wurde in beiden Gruppen gesehen. Zusammenfassung: Nach der Anwendung des zentralen Patellarsehnendrittels und des dreifachen/vierfachen ST-autografts fünf Jahre nach ACL-Rekonstruktion, waren die subjektiven und objektiven Resultate ähnlich. Außerdem wurde kein relevanter Unterschied in der Entnahmestellemorbidität zwischen den 2 Gruppen gefunden.
