Im Rahmen der vorliegenden Studie wurde an 220 Patienten, die zwischen 1988 und 2007 im König-Ludwig-Haus in Würzburg durch einen Operateur wegen rezidivierender, überwiegend posttraumatischer ventraler Schulterinstabilität offen oder arthroskopisch mittels (modifizierter) Bankart-Prozedur operiert wurden, der „Instability Severity Index Score (ISIS)“ so erhoben, wie er aus den präoperativen Unterlagen zu ermitteln war. Alle Patienten wurden nach postoperativen Rezidivluxationen befragt und die Schulterfunktion wurde mittels standardisiertem und validiertem Fragebogen durch den „Constant Score“ und den „Oxford Shoulder Instability Score“ untersucht.
Ziel der Studie war es, den von Balg und Boileau 2007 vorgestellten „Instability Severity Index Score“ (ISIS) auf seine Aussagekraft hin am vorliegenden Kollektiv zu überprüfen. Zeitgleich sollten ein Vergleich der offenen mit den arthroskopischen Stabilisierungen sowie eine Analyse der Ursachen der Rezidivluxationen erfolgen.
Insgesamt kam es in acht Fällen zu Rezidivluxationen (3,6 %). Die offen Operierten wiesen eine Rate von 3,1 %, die Gruppe der arthroskopisch Operierten 8,7 % Rezidive auf. Patienten mit weniger oder gleich sechs Punkten im ISIS hatten in 2,7 % Reluxationen, Patienten mit mehr als sechs Punkten in 8,1 %.
Patienten, die rückblickend gemäß der Empfehlung aus dem ISIS operiert wurden, hatten in 5,3 % Rezidivluxationen. Patienten, die entgegen der Empfehlung operiert wurden, in 3,5 %. Alle Unterschiede waren statistisch nicht signifikant. In allen Gruppen konnten in den funktionellen Scores sehr gute Ergebnisse mit durchschnittlich über 87 % im alters- und geschlechtsadaptierten Constant Score und über 42 Punkten im Oxford Shoulder Instability Score ohne signifikante Unterschiede erzielt werden. Von den insgesamt acht Patienten mit Reluxationen lagen von zwei Patienten CT-Untersuchungen nach aufgetretener Reluxation vor. In beiden Fällen konnten signifikante Glenoidranddefekte gefunden werden.
Aus Sicht der erhobenen Daten und der erzielten Ergebnisse ist der ISIS als nützlich zur präoperativen Risikobewertung sowie zur Entscheidung über das operative Vorgehen einzuschätzen, wobei er keine imperative Handlungsanweisung darstellen sollte. Die Empfehlung zum Korakoidtransfer nach Latarjet ab sieben Punkten im ISIS kann anhand dieser Daten nicht bestätigt werden. Vielmehr konnte gezeigt werden, dass eine offene Bankart-Operation mit selektivem Kapselshift sehr gute Langzeitergebnisse bezüglich der Reluxationsraten und der funktionellen Ergebnisse liefert. Im Hinblick auf die erzielten Ergebnisse und Fehleranalysen ist weiterhin festzuhalten, dass bei Verdacht auf einen Glenoiddefekt in der Regel eine CT mit 3D-Rekonstruktion und Seiten-vergleich erfolgen sollte, um die Indikation zum offenen Knochenblocktransfer nicht zu verpassen. Offene und arthroskopische Stabilisierungen können bei richtiger Indikationsstellung kurz- und mittelfristig vergleichbar gute Ergebnisse liefern. Langfristig aber scheint das minimal-invasive Vorgehen höhere Raten an Rezidivluxationen aufzuweisen. Wie auch in dieser Arbeit gezeigt werden konnte, ist ein langer Beobachtungszeitraum bei Studien, die das klinische Ergebnis von Schulterstabilisierungen untersuchen, sehr wichtig, um das wahre Ausmaß an postoperativen Rezidivinstabilitäten zu erfassen.
Fragestellung: Ziel der Untersuchung war es, Veränderungen im Verletzungsmuster im alpinen Skisport durch die Einführung des Carvingskis zu identifizieren. Methodik: Hierfür wurden 1383 Patienten, die in den Wintersaisons 2003/2005 und 2004/2005 die Ambulanz des Klinikums Garmisch-Partenkirchen aufsuchten, mit Hilfe eines standardisierten Fragebogens hinsichtlich unterschiedlicher, verletzungsrelevanter Parameter befragt und die Daten anschließend statistisch ausgewertet. Zusätzlich konnten im Rahmen eines Längsschnittsvergleiches Veränderungen im Vergleich zu den Wintern 2000/01 sowie 2001/02 eruiert werden. Ergebnisse: Signifikante Unterschiede im Sinne eines gehäuften Verletzungsrisikos ergaben sich für unterschiedliche Körperregionen für die verschiedenen Skitypen. Carvingskifahrer verletzten sich beispielsweise signifikant häufiger am Rumpf (p<0,05), jedoch seltener im Bereich des Kniegelenkes (p<0,05) und des Unterarms (p<0,01). Carvingskifahrer verletzten sich mit zunehmendem Fahrkönnen tendenziell häufiger im Bereich der oberen Extremität und des vorderen Kreuzbandes, während Kniegelenkstraumen insgesamt seltener beobachtet wurden. Kollisionsverletzungen waren bei Carvern nicht häufiger als bei Normalskifahrern zu beobachten. Die Verletzungsraten zeigten sich im longitudinalen Vergleich insgesamt weitgehend konstant. Schlussfolgerungen: Die verwendete Fahrtechnik (eigentliche carvende vs. driftende Fahrtechnik) scheint einen entscheidenden Einfluss auf das Verletzungsmuster auszuüben. Dies sowie die Auswirkungen des Carvingskis auf orthopädische Verletzungsmuster allgemein sollten auch weiterhin im Rahmen von Langzeitstudien beobachtet werden.
Osteozyten stehen vermehrt im Fokus als wesentliche Regulatoren der Knochenmineralisierung. Das ähnlich einem neuronalen Netzwerk aufgebaute lakunokanalikuläre Netzwerk der Osteozyten breitet sich im Knochen in drei Ebenen aus. Es wurde in dieser Arbeit ein 3D-Kollagengel-Modell verwendet und dort die Osteoblasten- bzw. Osteozytenzelllinien MLO-A5 und MLO-Y4, sowie humane mesenchymale Stammzellen aus Hüftköpfen eingebettet. Es wurden die optimalen Kulturbedingungen entwickelt und die Zellen über mehrere Wochen kultiviert, beobachtet und mit dem herkömmlichen 2D-Kulturmodell verglichen. MLO-A5 und MLO-Y4 bilden die zelltypischen Zellfortsätze. Die Gele kontrahieren, wenn hMSC und MLO-A5 eingebettet sind, mit MLO-Y4 zeigt sich über den gesamten Kultivierungszeitraum keinerlei Kontraktion der Kollagengele. Die Zellen wurden zudem osteogen differenziert und mit FGF23 und Klotho stimuliert. Es ergaben sich erste Hinweise auf eine FGF23 / Klotho-abhängige Inhibierung der lokalen Mineralisierung in osteogen differenzierten MLO-A5.
Es konnten einige osteogene Marker durch PCR und in den histologischen Schnitten mittels Antikörperfärbungen nachgewiesen werden, eindeutige Expressionsmuster und deren zeitliche Verläufe im Vergleich der osteogenen Differenzierungen und Zugabe von FGF23 und Klotho sind allerdings noch nicht identifizierbar und bedürfen womöglich höherer Fallzahlen und weiterer Untersuchungsmethoden. Insgesamt gesehen erweist sich das System aber als einfach und mit niederschwellig erreichbaren Methoden und Materialien durchzuführen.
Die primäre Gonarthrose ist ein komplexer, multifaktoriell bedingter Krankheitsprozess mit genetischer Komponente und gilt aufgrund ihrer epidemiologischen und sozioökonomischen Bedeutung als Volkskrankheit. Die Gene bzw. Proteine der Wnt- Familie spielen eine wichtige Rolle in der komplexen Regulation der Morphogenese, der Zelldifferenzierung , -proliferation und der Homöostase von Knochen und Knorpel. LRP 1 und LRP 5 fungieren als Co-rezeptoren des Wnt- Signalweges. In der vorliegenden Untersuchung wurden der A217V Polymorphismus des LRP 1 Gens und der A1330V Polymorphismus des LRP 5 Gens auf eine signifikante Assoziation mit dem Auftreten primärer Gonarthrose in einem arthroskopisch definierten Patientenkollektiv untersucht. Ziel der Studie war es, klar definierte Kollektive mit möglichst extremen Zuständen gegenüberzustellen und eine hochspezifische und hochsensitive Diagnostik zu verwenden. Junge, bereits in frühen Jahren an Gonarthrose erkrankte Patienten (<45 Jahre, Gruppe 1, n= 100) wurden älteren Patienten (>55 Jahre, Gruppe 3, n= 103), die nicht an Gonarthrose erkrankt waren, gegenübergestellt. Als weitere Gruppe dienten Patienten, welche eine endoprothetische Versorgung bei fortgeschrittener Gonarthrose erhielten (>55 Jahre, Gruppe 2, n= 86). Die Diagnose der primären Gonarthrose wurde anhand anamnestischer Daten sowie arthroskopischer bzw. direkt visuell erhobender Befunde im Rahmen der Arthrotomie bei Prothesenimplantation gestellt. Der Arthroseausschluss (Gruppe 3) erfolgte ebenfalls arthroskopisch. Methodisch wurden der WOMAC- Score, die Restriktions- Fragment- Längen- Polymorphismus (RFLP) Analyse sowie die Gensequenzierung angewendet. Die Allelverteilung der untersuchten Polymorphismen von LRP 1 und LRP 5 konnte zwischen den beiden gegenübergestellten Gruppen 1 und 3 keinen signifikanten Unterschied aufweisen. Für den LRP 5 A1330V Polymorphismus ergab sich ein signifikanter Unterschied (p< 0,05) zwischen den Gruppen 2 und 3. Dies kann als Hinweis auf eine mögliche arthrosefördernde oder auch protektive Funktion dieses Polymorphismus gewertet werden. Im Vergleich der Extremkollektive (Gruppe 1 vs. Gruppe 3) deutet sich lediglich ein Trend an (p= 0,067), so dass eine Assoziation des A1330V Polymorphismus des LRP 5 Gens mit primärer Gonarthrose erst durch weitere Untersuchungen mit höherer Fallzahl eindeutig bestätigt bzw. widerlegt werden kann.
Arthrose ist eine Erkrankung des Gelenks, die einen erheblichen Einfluss auf die Lebensqualität der Betroffenen sowie auch auf die Volkswirtschaft eines Landes hat. Das Kniegelenk ist das dabei am häufigsten befallene Gelenk. Leider gibt es bis zum heutigen Tag keine kausale, kurative Therapie dieser Erkrankung. In Frühstadien kann der Verlauf im Idealfall gestoppt werden, in Spätstadien ist häufig der künstliche Gelenkersatz die einzige Therapieoption, die Aussicht auf Schmerzfreiheit und einen gewissen Funktionserhalt gewährleisten kann. Lange Zeit war man der Meinung, dass die Arthrose eine logische Konsequenz des Alterns sei. Zuletzt geht man jedoch mehr und mehr davon aus, dass die Arthrose ein komplexes Zusamenspiel aus Entzündung, mechanischen Kräften, zellulären und biochemischen Prozessen sowie nicht zuletzt der Genetik ist. Die Genetik wurde in dieser Studie zum Fokus des Interesses gemacht, als ein entscheidender Faktor der primären Arthrose. Untersucht wurden Polymorphismen in entscheidenden Regionen (Promoter, Exons) von Genen, die im Arthrose-Prozess eine große Bedeutung haben: Il-1 alpha, Il-1 beta, TNF alpha und MMP-3 (Stromelysin). Viele bereits durchgeführte Studien lieferten widersprüchliche Ergebnisse, wiesen zu geringe Fallzahlen auf oder definierten die Kollektive nur unzureichend. Ziel dieser Studie war es klar definierte Kollektive mit möglichst extremen Zuständen gegenüberzustellen. Junge bereits in frühen Jahren an Arthrose Erkrankte (<45 Jahre) sollten gesunden Patienten über 55 Jahre gegenübergestellt werden. Als dritte Gruppe dienten Altersarthrotiker. Die Diagnosen wurden arthroskopisch gestellt, der Zielwert pro Gruppe sollte 100 Patienten betragen. Für die Auswertung wurden etablierte Verfahren wie die PCR, Restriktion und der Gensequencer genutzt. Die Allelverteilungen der Polymorphismen TNF alpha und MMP-3 konnten in der Auswertung zwischen den Gruppen keinen signifikanten Unterschied aufweisen. Das legt nahe, dass diese beiden Polymorphismen wohl keinen Einfluss auf die Entstehung von Arthrose haben. Bei Il-1 alpha und Il-1 beta wurden keine eindeutigen Ergebnisse erzielt. Es deutet sich ein Trend an, dass diese beiden Polymorphismen einen Einfluss haben, der allerdings erst durch eine höhere Fallzahl bestätigt bzw. widerlegt werden könnte. Ziel weiterer Untersuchungen sollte es ebenso sein, die bestehende bzw. die in der Fallzahl erhöhte DNS-Bank auf weitere Polymorphismen von Genen, die im Arthroseprozess eine Rolle spielen (z.B. Il-1 ra, weitere MMPs, TIMP etc.), zu untersuchen. Interessant könnte auch die Kombination einzelner Varianten innerhalb eines Genclusters sein, entsprechend dem Beispiel von Smith, A.J. et al. 2004 bei einem Il-1 R1-Il-1 alpha –Il-1 beta-Il-1 RN Gencluster. Sicherlich bedarf es noch viel intensiver Forschung, um das komplexe Zusammenspiel der einzelnen Faktoren im Arthroseprozess zu verstehen, um so die einzelnen Mosaiksteinchen innerhalb des Gesamtkomplexes darzustellen. Jedoch lässt sich nur auf diese Weise das Ziel erreichen, eines Tages diese multifaktorielle Erkrankung zu verstehen und zu heilen.
Die AC-Sprengung ist eine weit verbreitete Verletzung des Schultergürtels und am häufigsten auf einen Sportunfall vorwiegend junger Männer zurückzuführen. Die optimale Therapie wird kontrovers diskutiert, keine der rund 150 OP-Methoden hat sich bisher als den anderen überlegen erwiesen. Allerdings gelten Operationen, bei denen das AC-Gelenk temporär durch eine intramedulläre Schienung per Kirschner-Draht ruhiggestellt wird, als sehr zuverlässig. Sie sind aufgrund einer sehr variablen Anatomie jedoch auch für erfahrene Operateure technisch anspruchsvoll. Ziel dieser Arbeit ist es daher, mithilfe der gewonnenen Kenntnisse zur idealen Lage eines K-Drahtes das operative Vorgehen zur temporären Transfixation des ACGs künftig durch eine gezieltere Platzierung zu erleichtern, somit die Operationsmethode zu optimieren und folglich das Outcome zu verbessern.
Für diese Arbeit wurden bereits vorliegende anonymisierte computertomographische Daten gesunder AC-Gelenke nach ihrer physiologischen Anatomie sowie der Lage eines virtuell ideal platzierten transartikulären K-Drahtes ausgewertet. Hierfür wurden CT-Daten von insgesamt 66 Schultern herangezogen, der Epidemiologie der AC-Sprengung entsprechend waren hiervon 59 Patienten und 7 Patientinnen zuzuordnen.
Die erhobenen Daten zeigen, dass die Lage des Eintrittspunktes idealerweise durch den orthogonalen Abstand des Drahtes zur Acromionspitze definiert wird und durchschnittlich 12,89 mm beträgt. Er ist primär abhängig von der Körpergröße und kann daher präoperativ anhand einer Regressionsgeraden individuell für jeden Patienten bestimmt werden.
Der Drahtverlauf sollte primär durch Zielen auf den markierten Austrittspunkt definiert werden. Das Abschätzen der Bohrrichtung anhand von Winkeln erscheint nahezu unmöglich.
Die Drahtlänge beläuft sich im Mittel auf 58,06 mm. Je kleiner der AC-Winkel ist, desto steiler und auch kürzer zeigt sich der Drahtverlauf.
Die Lage des Austrittspunktes korreliert ebenfalls signifikant mit dem AC-Winkel und kann daher nach erfolgter Winkelmessung im Röntgenbild anhand einer Regressionsgeraden abgelesen werden. Der mithilfe der Daten ermittelte Austrittspunkt eines ideal platzierten K-Drahtes befindet sich durchschnittlich auf Höhe des lateralen Claviculawinkels und somit auf Höhe der CC-Bänder.
Bei der nur geringen Fallzahl weiblicher Patienten besteht eine noch eingeschränkte Aussagekraft bezüglich geschlechtsabhängiger Lageunterschiede. Nach bisher vorliegenden Daten kann eine geschlechtsunabhängige OP-Planung erfolgen. Relevante ACG-Typ abhängige Lageunterschiede konnten ebenfalls nicht festgestellt werden, eine präoperative Bestimmung des anatomischen ACG-Typs ist daher nicht erforderlich.
Die erhobenen Daten deuten darauf hin, dass die ideale Drahtplatzierung unter Einhaltung aller Drahtlagekriterien nicht immer möglich ist. Betroffen sind kleine Patienten (Grenzwert: 158,6 cm Körpergröße), bei denen der Mindestabstand zur Acromionspitze nicht sichergestellt werden kann. Zudem besteht bei Patienten mit einem kleinen AC-Winkel (Grenzwert: 156,2°) das Risiko, die Mindestdrahtlänge innerhalb der Clavicula zu unterschreiten. In diesen Fällen muss entweder dezent von der idealen Drahtlage abgewichen oder auf ein alternatives OP-Verfahren ausgewichen werden.
Die Resektion der lateralen Klavikula ist eine häufig durchgeführte Operation bei symptomatischer AC-Gelenkarthrose. Es lagen bislang allerdings keine Daten vor, in welchem Abstand zum AC- Gelenk die Ligg. coracoclavicularia an der Klavikula inserieren. In vorliegender Studie wurden 36 Schulterpräparate, 18 von Männern und 18 von Frauen untersucht. Es wurde der Abstand der AC- Gelenkkapsel, des Lig. trapezoideum und des Lig conoideum zum AC-Gelenk gemessen. Zusätzlich wurde der Abstand des N. suprascapularis aufgrund seiner Nähe zum AC-Gelenk an der Incisura scapula und am Lig. spinoglenoidale zum AC- Gelenk gemessen. Der Abstand der an der Klavikula inserierenden Kapselfasern zum AC-Gelenk betrug im Durchschnitt bei Frauen 0,7 cm und bei Männern 0,8 cm. Die Fasern des Lig. trapezoideum an der Klavikula waren durchschnittlich 0,9 – 2,4 cm bei Frauen und 1,0 – 2,8 cm bei Männern vom distalen Klavikulaende entfernt. Die an der Klavikula inserierenden Fasern des Lig. conoideum lagen durchschnittlich bei Frauen 2,3 – 4,4 cm und bei Männern 2,9 – 5,0 cm vom lateralen Klavikulaende entfernt. Der N. suprascapularis verlief an der Incisura scapula durchschnittlich im Abstand von 3,8 cm und am Lig. spinoglenoidale durchschnittlich im Abstand von 3,3 cm zum AC-Gelenk. Es besteht also ein ausreichender Sicherheitsabstand zum Nerv bei der empfohlenen Resektionsweite von maximal 1 cm. Bei einer Resektion der lateralen Klavikula von 1 cm würden im Durchschnitt 8 % des Lig. trapezoideum durchtrennt werden. Bei einer Resektionsweite von 2 cm wären bereits 63 % des Lig. trapezoideum und bei einer Resektionsweite von 1 inch (2,54 cm) 90 % durchtrennt. Daher ist eine Resektionsweite der lateralen Klavikula von maximal 0,8 cm bei Frauen und maximal 1,0 cm bei Männern zu empfehlen. Falls sich bei Bewegungen wie Hyperabduktion und Hyperadduktion immer noch Knochenkontakt zwischen Akromion und Klavikula zeigt, sollten auch wenige Millimeter vom medialen Akromion reseziert werden. Bei einer größeren Resektionsweite an der lateralen Klavikula wäre eine Instabilität und damit verbundene Beschwerden möglich.
Handgelenkserkrankungen haben sowohl in Amerika wie auch in Europa mit steigenden Fallzahlen einen durch den Ausfall der Arbeitskraft nicht unerheblichen Einfluß auf die Volkswirtschaft. Ständige Wiederholungen in hoher Frequenz werden für die RSI ursächlich verantwortlich gemacht. Basierend auf der Annahme, dass Vibrationen und Beschleunigungskräften mittels Dämpfkörpern beeinflusst werden können, wurde die Coopercare Lastrap-Bandage entwickelt. Obwohl die Behandlung mit Handgelenksorthesen eine allgemein gängige Form der Behandlung von verschiedenen Handgelenkserkrankungen darstellt, gibt es hierzu nur sehr wenige klinische Daten über den Stellenwert dieser Verfahren. Daher wurde in einer prospektiven randomisierten Längsschnitt-Studie der Stellenwert einer Bandagenbehandlung mit biomechanisch begründetem Ansatz im Vergleich zur konventionellen Bandagentherapie an 34 Patienten mit unterschiedlichen Erkrankungen des Handgelenks getestet. Unserer Studie zufolge sind entsprechend dem biomechanischen Ansatz die unter 40-jährigen männlichen Patienten mit seit kurzem bestehender Tendovaginitis die Zielgruppe, die am Besten von einer Bandagentherapie mit der Lastrap®-Bandage profitieren. Bei unter 40-jährigen männlichen Patienten mit Distorsion des Handgelenks ist die Manu-Hit®-Bandage zu bevorzugen. Durch die deutliche Schmerzreduktion können hier Handgelenksorthesen unter anderem den Gebrauch von NSAR verringern und damit die Arzneimittelausgaben senken.
In bone tissue engineering, the design of 3D systems capable of recreating composition, architecture and micromechanical environment of the native extracellular matrix (ECM) is still a challenge. While perfusion bioreactors have been proposed as potential tool to apply biomechanical stimuli, its use has been limited to a low number of biomaterials. In this work, we propose the culture of human mesenchymal stem cells (hMSC) in biomimetic mineralized recombinant collagen scaffolds with a perfusion bioreactor to simultaneously provide biochemical and biophysical cues guiding stem cell fate. The scaffolds were fabricated by mineralization of recombinant collagen in the presence of magnesium (RCP.MgAp). The organic matrix was homogeneously mineralized with apatite nanocrystals, similar in composition to those found in bone. X-Ray microtomography images revealed isotropic porous structure with optimum porosity for cell ingrowth. In fact, an optimal cell repopulation through the entire scaffolds was obtained after 1 day of dynamic seeding in the bioreactor. Remarkably, RCP.MgAp scaffolds exhibited higher cell viability and a clear trend of up-regulation of osteogenic genes than control (non-mineralized) scaffolds. Results demonstrate the potential of the combination of biomimetic mineralization of recombinant collagen in presence of magnesium and dynamic culture of hMSC as a promising strategy to closely mimic bone ECM.
Bisphenol A-related effects on bone morphology and biomechanical properties in an animal model

Bisphenol A (BPA), which is contained in numerous plastic products, is known to act as an endocrine-disruptive, toxic, and carcinogenic chemical. This experimental series sought to determine the influence of BPA exposure on the femoral bone architecture and biomechanical properties of male and female Wistar rats. BPA was applied subcutaneously by using osmotic pumps. After 12 weeks, the bones were analyzed by micro-computed tomography (micro-CT) and a three-point bending test. Comparing the female low- and high-dose groups, a significantly greater marrow area (p = 0.047) was identified in the group exposed to a higher BPA concentration. In addition, the trabecular number tended to be higher in the female high-dose group when compared to the low-dose group (p > 0.05). The area moment of inertia also tended to be higher in the male high-dose group when compared to the male low-dose group (p > 0.05). Considering our results, BPA-related effects on the bone morphology in female Wistar rats are osteoanabolic after high-dose exposure, while, in male rats, a tendency toward negative effects on the bone morphology in terms of a reduced cross-sectional cortical area and total area could be demonstrated.
Active vitamin D (1,25(OH)2D3) is known to exert direct anti-cancer actions on various malignant tissues through binding to the vitamin D receptor (VDR). These effects have been demonstrated in breast, prostate, renal and thyroid cancers, which all have a high propensity to metastasise to bone. In addition, there is evidence that vitamin D catabolism via 24-hydroxylase (CYP24A1) is altered in tumour cells, thus, reducing local active vitamin D levels in cancer cells. The aim of this study was to assess VDR and CYP24A1 expression in various types of bone metastases by using immunohistochemistry. Overall, a high total VDR protein expression was detected in 59% of cases (39/66). There was a non-significant trend of high-grade tumours towards the low nuclear VDR expression (p = 0.07). Notably, patients with further distant metastases had a reduced nuclear VDR expression (p = 0.03). Furthermore, a high CYP24A1 expression was detected in 59% (39/66) of bone metastases. There was a significant positive correlation between nuclear VDR and CYP24A1 expression (p = 0.001). Collectively, the VDR and CYP24A1 were widely expressed in a multitude of bone metastases, pointing to a potential role of vitamin D signalling in cancer progression. This is of high clinical relevance, as vitamin D deficiency is frequent in patients with bone metastases.
Summary
In adult hypophosphatasia (HPP) patients, elevated lumbar spine dual X-ray absorptiometry (DXA) values are associated with markers of disease severity and disease-specific fracture risk while femoral bone mineral density (BMD), being largely unaffected by the disease severity, may still be useful to monitor other causes of increased fracture risk due to low BMD.
Introduction
Hypophosphatasia (HPP) is a rare inherited metabolic disorder due to deficient activity of the tissue-nonspecific alkaline phosphatase (TNAP). Clinical manifestation in adult HPP patients is manifold including an increased risk for fractures, but data regarding clinical significance of DXA measurement and associations with fracture risk and disease severity is scarce.
Methods
Retrospective single-center analysis of DXA scans in patients with confirmed HPP (documented mutation, clinical symptoms, low alkaline phosphatase activity). Further data evaluation included disease-related fractures, laboratory results (alkaline phosphatase, pyridoxalphosphate, phosphoethanolamine), and medical history.
Results
Analysis included 110 patients (84 female, mean age of 46.2 years) of whom 37.3% (n = 41) were harboring two mutations. Average T-Score level at the lumbar spine was − 0.1 (SD 1.9), and mean total hip T-Score was − 1.07 (SD 0.15). Both lower ALP activity and higher substrate levels (pyridoxalphosphate and phosphoethanolamine) were significantly correlated with increased lumbar spine T-Score levels (p < 0.001) while BMD at the hip was not affected by indicators of disease severity. Increased lumbar spine BMD was significantly associated with an increased risk for HPP-related fractures, prevalent in 22 (20%) patients (p < 0.001) with 21 of them having biallelic mutations.
Conclusion
BMD in adult HPP patients is not systematically reduced. Conversely, increased lumbar spine BMD appears to be associated with severely compromised mineralization and increased risk for HPP-related fractures while BMD at the hip appears unaffected by indicators of disease severity, suggesting suitability of this anatomic location for assessing and discerning disorders with increased fracture risk owing to reduced BMD like osteoporosis.

Summary
There is limited understanding of how asfotase alfa affects mineral metabolism and bone turnover in adults with pediatric-onset hypophosphatasia. This study showed that adults with hypophosphatasia treated with asfotase alfa experienced significant changes in biochemical markers of bone and mineral metabolism, possibly reflecting enhanced bone remodeling of previously osteomalacic bone.
Introduction
Hypophosphatasia (HPP), due to a tissue nonspecific alkaline phosphatase (TNSALP) deficiency, can cause impaired bone mineralization and turnover. Although HPP may be treated with asfotase alfa, an enzyme replacement therapy, limited data are available on how treatment with asfotase alfa affects mineral metabolism and bone turnover in adults with HPP.
Methods
ALP substrates, bone turnover and mineral metabolism markers, and bone mineral density (BMD) data from EmPATHY, a single-center, observational study of adults (≥ 18 years) with pediatric-onset HPP treated with asfotase alfa (NCT03418389), were collected during routine clinical care and analyzed from baseline through 24 months of treatment.
Results
Data from 21 patients showed significantly increased ALP activity and reduced urine phosphoethanolamine (PEA)/creatinine (Cr) ratios after baseline through 24 months of asfotase alfa treatment. There were significant transient increases in parathyroid hormone 1-84 (PTH), osteocalcin, and procollagen type 1 N-propeptide (P1NP) levels at 3 and 6 months and in tartrate-resistant acid phosphatase 5b (TRAP5b) levels at 3 months, with a significant decrease in N-terminal telopeptide of type 1 collagen (NTX) levels at 24 months. Lumbar spine BMD T scores continuously increased during treatment.
Conclusion
Significant changes in bone turnover and mineral metabolism markers after asfotase alfa treatment suggest that treatment-mediated mineralization may enable remodeling and bone turnover on previously unmineralized surfaces. Urine PEA/Cr ratios may be a useful parameter in monitoring treatment during routine care.
Hypophosphatasia (HPP) is a rare, inherited, metabolic disease caused by deficient tissue non‐specific alkaline phosphatase activity. This study aims to assess patient‐reported pain, disability and health‐related quality of life (HRQoL) in a real‐world cohort of adults with HPP who were not receiving asfotase alfa during the analysis. Adults (≥18 years old) with HPP (confirmed by ALPL gene mutation and/or low serum alkaline phosphatase activity for age/sex) were identified from the Global HPP Registry (NCT02306720). Demographics, clinical characteristics, and data on patient‐reported pain, disability, and HRQoL (assessed by Brief Pain Inventory Short Form [BPI‐SF], Health Assessment Questionnaire Disability Index [HAQ‐DI], and 36‐Item Short‐Form Health Survey version 2 [SF‐36v2], respectively) were stratified by pediatric‐ and adult‐onset HPP and summarized descriptively. Of the 304 adults included (median [min, max] age 48.6 [18.8, 79.8] years; 74% women), 45% had adult‐onset HPP and 33% had pediatric‐onset HPP (unknown age of onset, 22%). Of those with data, 38% had experienced ≥5 HPP manifestations and 62% had a history of ≥1 fracture/pseudofracture. Median (Q1, Q3) BPI‐SF scores were 3.5 (1.5, 5.3) for pain severity and 3.3 (0.9, 6.2) for pain interference. Median (Q1, Q3) disability on the HAQ‐DI was 0.3 (0.0, 0.7). Median (Q1, Q3) physical and mental component summary scores on the SF‐36v2 were 42.4 (32.7, 49.9) and 45.3 (36.3, 54.8), respectively. Greater numbers of HPP manifestations experienced/body systems affected correlated significantly with poorer scores on the BPI‐SF, HAQ‐DI, and SF‐36v2 (all p < 0.05). No significant differences between adults with pediatric‐ and adult‐onset HPP were observed for patient‐reported outcomes, except for disability and the BPI‐SF question “pain at its worst,” which were significantly higher among adults with pediatric‐ versus adult‐onset HPP (p = 0.03 and 0.04, respectively). These data from the Global HPP Registry show that adults with HPP have a substantial burden of illness that is associated with reduced patient‐reported HRQoL, regardless of age of disease onset.
Juvenile Dermatomyositis (JDM) is a rare autoimmune disease in children and adolescents. In these patients calcinosis might be the most characteristic symptom. However there are only few reported cases of intramuscular calcinosis in Dermatomyositis. We report a case of calcinosis universalis (CU) of the elbow in JDM successfully treated with broaching. The patient, a 24-year-old woman, suffered from a long history of JDM. On examination she presented with a fistula lateral to the olecranon and pain of the right elbow joint. Plain X-rays displayed a diffuse pattern of multiple periarticular, subcutaneous, and intramuscular calcifications. The patient underwent surgery for histological and microbiological sampling as well as broaching. Intraoperatively sinus formation and subfascial hard calcium deposition were found. Due to the risk of collateral tissue damage, incomplete broaching was performed. A local infection with Staphylococcus was diagnosed and treated with antibiotics. On six-week and 30-month follow-up the patient was free of pain and had very good function. Calcifications on standard radiographs had almost resolved entirely. This case report gives a summary on calcinosis in Dermatomyositis and adds a new case of recalcitrant CU to the literature. Broaching surgery proved to be a reliable treatment option in symptomatic calcinosis.
Resection of musculoskeletal sarcoma can result in large bone defects where regeneration is needed in a quantity far beyond the normal potential of self-healing. In many cases, these defects exhibit a limited intrinsic regenerative potential due to an adjuvant therapeutic regimen, seroma, or infection. Therefore, reconstruction of these defects is still one of the most demanding procedures in orthopaedic surgery. The constraints of common treatment strategies have triggered a need for new therapeutic concepts to design and engineer unparalleled structural and functioning bone grafts. To satisfy the need for long-term repair and good clinical outcome, a paradigm shift is needed from methods to replace tissues with inert medical devices to more biological approaches that focus on the repair and reconstruction of tissue structure and function. It is within this context that the field of bone tissue engineering can offer solutions to be implemented into surgical therapy concepts after resection of bone and soft tissue sarcoma. In this paper we will discuss the implementation of tissue engineering concepts into the clinical field of orthopaedic oncology.
Purpose
The AMADEUS (Area Measurement And DEpth and Underlying Structures) scoring and grading system has been proposed for the MRI based evaluation of untreated focal chondral defects around the knee. The clinical practicability, its correlation with arthroscopically assessed grading systems (ICRS – International Cartilage Repair Society) and thereby its clinical value in terms of decision making and guiding prognosis was yet to determine.
Methods
From 2008 to 2019 a total of 89 individuals were indicated for high tibial valgus osteotomy (HTO) due to tibial varus deformity and concomitant chondral defects of the medial compartment of the knee. All patients received a preoperative MRI (1.5 Tesla or 3.0 Tesla) and pre-osteotomy diagnostic arthroscopy. Chondral defects of the medial compartment were scored and graded with the MRI based AMADEUS by three independent raters and compared to arthroscopic defect grading by the ICRS system. Interrater and intrarater reliability as well as correlation analysis with the ICRS classification system were assessed.
Results
Intraclass correlation coefficients for the various subscores of the AMADEUS showed an overall good to excellent interrater agreement (min: 0.26, max: 0.80). Intrarater agreement turned out to be substantially inferior (min: 0.08, max: 0.53). Spearman correlation revealed an overall moderate correlative association of the AMADEUS subscores with the ICRS classification system, apart from the defect area subscore. Sensitivity of the AMADEUS to accurately identify defect severity according to the ICRS was 0.7 (0.69 for 3.0 Tesla MRI, 0.67 for 1.5 Tesla MRI). The mean AMADEUS grade was 2.60 ± 0.81 and the mean ICRS score 2.90 ± 0.63.
Conclusions
Overall, the AMADEUS with all its subscores shows moderate correlation with the arthroscopic chondral grading system according to ICRS. This suggests that chondral defect grading by means of the MRI based AMADEUS is well capable of influencing and guiding treatment decisions. Interrater reliability shows overall good agreement.
