Chronic respiratory diseases are among the leading causes of death worldwide, but only symptomatic therapies are available for terminal illness. This in part reflects a lack of biomimetic in vitro models that can imitate the complex environment and physiology of the lung. Here, a copolymeric membrane consisting of poly(ε‐)caprolactone and gelatin with tunable properties, resembling the main characteristics of the alveolar basement membrane is introduced. The thin bioinspired membrane (≤5 μm) is stretchable (up to 25% linear strain) with appropriate surface wettability and porosity for culturing lung epithelial cells under air–liquid interface conditions. The unique biphasic concept of this membrane provides optimum characteristics for initial cell growth (phase I) and then switch to biomimetic properties for cyclic cell‐stretch experiments (phase II). It is showed that physiologic cyclic mechanical stretch improves formation of F‐actin cytoskeleton filaments and tight junctions while non‐physiologic over‐stretch induces cell apoptosis, activates inflammatory response (IL‐8), and impairs epithelial barrier integrity. It is also demonstrated that cyclic physiologic stretch can enhance the cellular uptake of nanoparticles. Since this membrane offers considerable advantages over currently used membranes, it may lead the way to more biomimetic in vitro models of the lung for translation of in vitro response studies into clinical outcome.
We aimed to elucidate the diagnostic potential of the C-X-C motif chemokine receptor 4 (CXCR4)-directed positron emission tomography (PET) tracer \(^{68}\)Ga-Pentixafor in patients with poorly differentiated neuroendocrine carcinomas (NEC), relative to the established reference standard \(^{18}\)F-FDG PET/computed tomography (CT). In our database, we retrospectively identified 11 treatment-naïve patients with histologically proven NEC, who underwent \(^{18}\)F-FDG and CXCR4-directed PET/CT for staging and therapy planning. The images were analyzed on a per-patient and per-lesion basis and compared to immunohistochemical staining (IHC) of CXCR4 from PET-guided biopsies. \(^{68}\)Ga-Pentixafor visualized tumor lesions in 10/11 subjects, while \(^{18}\)F-FDG revealed sites of disease in all 11 patients. Although weak to moderate CXCR4 expression could be corroborated by IHC in 10/11 cases, \(^{18}\)F-FDG PET/CT detected significantly more tumor lesions (102 vs. 42; total lesions, n = 107; p < 0.001). Semi-quantitative analysis revealed markedly higher 18F-FDG uptake as compared to \(^{68}\)Ga-Pentixafor (maximum and mean standardized uptake values (SUV) and tumor-to-background ratios (TBR) of cancerous lesions, SUVmax: 12.8 ± 9.8 vs. 5.2 ± 3.7; SUVmean: 7.4 ± 5.4 vs. 3.1 ± 3.2, p < 0.001; and, TBR 7.2 ± 7.9 vs. 3.4 ± 3.0, p < 0.001). Non-invasive imaging of CXCR4 expression in NEC is inferior to the reference standard \(^{18}\)F-FDG PET/CT.
Prädiktion des Verschlusses großer intrakranieller Arterien anhand präklinischer Schlaganfallscores

2015 konnte in mehreren Studien ESCAPE, EXTENDED IA, MR CLEAN, REVASCAT, SWIFT-PRIME eine signifikante Überlegenheit der mechanischen Thrombektomie verglichen mit der alleinigen i. v. Lysetherapie mit rtPA bezogen auf Revaskularisierung bei Patienten mit einer LVO (large vessel occlusion) nachgewiesen werden. Diese neue Therapiemöglichkeit erforderte eine Aufteilung der Patienten die von einer Thrombektomie profitieren (LVO) und der Patienten, die keiner Thrombektomie zugeführt werden können (nLVO). Die zentrale Fragestellung der Studie ist: Kann ein symptomorientierter Schlaganfallscore die Wahrscheinlichkeit eines großen intrakraniellen Gefäßverschlusses mit hinreichender Präzision vorhersagen und kann auf Basis dieser Vorhersage ein Patient direkt in ein übergeordnetes Schlagfanfallzentrum gebracht werden, obwohl sich dadurch eine Bridging Lysetherapie verzögern würde?
Um diesen Fragen auf den Grund zu gehen führten wir eine monozentrische Querschnittstudie durch, in deren Rahmen 215 Patienten rekrutiert wurden. Die Rekrutierung erfolgte mittels eines aus Subitems bereits etablierter Schlafanfallscores (FAST, CPSS, LAPSS, 3ISS, RACE), zusammengesetzten Fragebogens. Die ausgefüllten Fragebögen wurde in Excel digitalisiert und mittels SPSS, Signifikanz und Odds Ratio berechnet. Anschließend wurde aus den signifikanten Subitems mit der höchsten Odds Ratio ein neuer einfach anzuwendender Schlaganfallscore, bestehend aus den präklinisch erhobenen Daten gebildet (Würzburg Score of Large Vessel Occlusions, WOLVE- Score). Weiter wurden Signifikanz, Odds Ratio, Sensitivität und Spezifität des WOLVE-Score mit denen der oben genannten etablieren Scores verglichen.
Das postoperativ neu aufgetretene Vorhofflimmern (POAF) ist die häufigste Herzrhythmusstörung nach einer Herzoperation und mit einer erhöhten Morbidität und Mortalität verbunden. Das Ziel der Studie war fibrotische und inflammatorische Biomarker sowie die kalziuminduzierte Kontraktionskraft im linken und rechten Vorhof als Prädiktoren für POAF zu bewerten.
Von den 229 eingeschlossenen Patienten im Sinusrhythmus, die sich einer elektiven Herzbypass-Operation unterzogen haben entwickelten 38 Patienten ein POAF. Klinische und laborchemische Daten sowie echokardiographische Befunde wurden erhoben und kalziuminduzierte Kraftmessungen von geskinnten linken und rechten atrialen Muskelfasern durchgeführt.
Patienten mit POAF waren älter, hatten signifikant größere LA-Fläche und RA-Fläche, eine höhere Prävalenz der arteriellen Hypertonie und signifikant geringere TAPSE-Werte. Von den getesteten Biomarkern für Fibrose und Entzündung waren MMP-9 und Pentraxin-3 signifikant verringert und NT-Pro-BNP und GDF-15 signifikant erhöht.
Patienten mit POAF hatten zusätzlich signifikant niedrigere LA-Kraftwerte (pCa 5,5 bis 4,52; p <0,04) und RA-Kraftwerte (pCa 5,0 bis pCa 4,52; p <0,04). Die univariate Analyse ergab den LA-Kraftwert bei pCa 5,5 (p = 0,033), das fortgeschrittene Alter (p = 0,033), die LA-Fläche (p = 0,013), die RA-Fläche (p = 0,081) und das TAPSE (p = 0,01) als unabhängige Prädiktoren für POAF.
Die Berücksichtigung dieser Parameter könnte die Identifizierung und Risikostratifizierung von Patienten mit POAF-Risiko unterstützen.
Introduction: Abdominal aortic aneurysm (AAA) is a pathological saccular enlargement most often of the infrarenal aorta. Eventual rupture is fatal, making preemptive surgical therapy upon a diameter threshold of >50mm the treatment of choice. The pathophysiology, especially the initial trigger aortic remodeling is still largely unknown. However, some characteristic features involved in aneurysm growth have been established, such as medial angiogenesis, low-grade inflammation, vascular smooth muscle cell (VSMC) phenotype switch, extracellular remodeling, altered hemodynamics and an eventual humoral immune answer. Currently, no medical treatment options are available. RNA therapeutics and drug repurposing offer new possibilities to overcome this shortage. Using such to target angiogenesis in the aneurysm wall and investigate their potential mechanisms is the aim of this thesis. Material and Methods: We test our hypothesis by targeting the long non-coding RNA H19 and re-use the anti-cancer drug Lenvatinib in two murine inducible AAA models and one preclinical large animal model in the LDLR-/- pig. Furthermore, a H19-/- mouse is included to verify the results. AAA and control samples from a human biobank along with a primary human cell culture are used to verify results ex vivo by qPCR, WesternBlot, live cell imaging, histo- and immunohistochemistry along with gene array analysis, RNA knockdown, pull-down- and promotor assays. Results: H19 is significantly upregulated in AAA mice models and its knockdown limited aneurysm growth. It is well known that H19 interacts with several transcription factors. We found that cytoplasmic interaction between H19 and hypoxia-inducible factor 1-alpha (HIF1α) increased apoptosis in cultured SMCs associated with sequential p53 stabilization. In contrast, the knockdown of H19 was associated with markedly decreased apoptotic cell rates. Our data underline that HIF1α was essential in mediating the pro-apoptotic effects of H19. Secondly, Lenvatinib was applied both systemically and locally by endovascular means in mice with an established AAA. The drug significantly halted aneurysm growth and array analysis revealed myosin heavy chain 11 (MYH11) as the most differentially regulated target. This was shown to be up regulated after Lenvatinib treatment of primary AAA smooth muscle cells suggesting a salvage mechanism to obtain a contractile phenotype based on gene expression and immunohistochemistry. The same results were shown upon a local endovascular Lenvatinib-coated balloon angioplasty in the established aneurysmatic lesion of a novel atherosclerotic LDLR-/- Yucatan minipig model. Decreased phosphorylation of extracellular-signal regulated kinases 1-2 (ERK1-2) is the downstream effect of Lenvatinib-specific blockage of the vascular endothelial growth factor receptor (VEGFR2). Conclusion: Taking into account the heterogeneity of the disease, inhibition of VSMC phenotype switch, extracellular remodeling and angiogenesis seem promising targets in some if not all AAA patients. Together with surveillance and surgical therapy, these new non-invasive treatment strategies would allow for a more personalized approach to treat this disease.
Testung verschiedener Strategien für die Regeneration von Knorpeldefekten im Ex vivo-Testsystem

Die Degeneration des Gelenkknorpels ist Hauptursache für chronische Schmerzen und eine dadurch bedingte Einschränkung der Lebensqualität. Für die Sozialversicherungssysteme ist dies mit steigenden Kosten verbunden. Gegenwärtige Behandlungsoptionen wie die Mikrofrakturierung oder die (matrix-assoziierte) Autologe Chondrozytentransplantation (M-) ACT führen zu einem minderwertigen Reparaturgewebe aus Faserknorpel mit unzureichenden mechanischen Eigenschaften an der Defektstelle. Es besteht ein Bedarf an der Entwicklung und Testung neuer Knorpeltherapien, die ein funktionelles Reparaturgewebe für nachhaltige Beschwerdefreiheit erzeugen. Das hier verwendete kürzlich etablierte osteochondrale Ex vivo-Testsystem (EVTS) eignet sich zur Evaluation unterschiedlicher zellbasierter Behandlungsansätze für die Knorpelregeneration.
Aus der medialen Femurkondyle von Schweinen wurden zylindrische 8 mm große osteochondrale Explantate (OCE) isoliert. Es wurden Knorpel-Knochendefekte und reine Knorpeldefekte kreiert und mit autologen Schweine-Chondrozyten (CZ) bzw. einer Mischung aus CZ und mesenchymalen Stammzellen (MSC) gefüllt, die in Kollagen Typ I Hydrogel eingebettet waren. Nach vierwöchiger Kultivierung wurden die Proben histologisch und immunhistochemisch gefärbt (Safranin-O-Färbung, Kollagen Typ II, Aggrekan), die Zellvitalität (Lebend-Tot-Färbung) überprüft und die extrazelluläre Matrixproduktion analysiert. Nach vierwöchiger Kultur im EVTS in Normoxie und Hypoxie zeigten sich die in Kollagen-I-Hydrogel eingebetteten Zellen lebensfähig. Die Auswertung der verschiedenen Ansätze erfolgte über den standardisierten ICRS-II-Score der International Cartilage Repair Society (ICRS) mit drei unabhängigen Bewertern. Insgesamt resultierten bessere Ergebnisse im Hinblick auf die Matrixsynthese in den Monokulturen aus CZ im Vergleich zu den Co-Kulturen aus CZ und MSCs. Da dieser Unterschied nicht groß war, könnten MSCs zur Einsparung autologer CZ eine Alternative in der Behandlung von Knorpeldefekten darstellen. Hypoxie spielte eine Rolle bei reinen Knorpeldefekten, nicht bei Knorpel-Knochendefekten. Dies bestätigt die Bedeutung des physiologischen hypoxischen Milieus des Gelenkknorpels, das einen niedrigen Sauerstoffgehalt von 2-5
VII
% aufweist. Die Ergebnisse zeigen, dass die unterschiedlichen Faktoren aus Zellkombination, Knorpeldefektgröße und Kultivierung in Hypoxie oder Normoxie Einfluss auf die Ausbildung der extrazellulären Matrix haben. Weiterhin fehlt jedoch das Verständnis für die genauen Mechanismen des Knorpelregenerationsverhaltens. Ex vivo-Testsysteme können dabei helfen ein weiteres Verständnis zu erlangen und entsprechende Behandlungsstrategien zu evaluieren.
Investigations into the Pathogenic Antibody-Antigen-Interference of Glycine Receptor Autoantibodies

Anti-glycine receptor (GlyR) autoantibodies belong to the novel group of autoantibodies that target neuronal cell-surface antigens (NCS), which are accompanied with various neurologic and neuropsychiatric conditions. The inhibitory ionotropic GlyR is one of the major inhibitory neurotransmitter receptors and therefore involved in maintaining homeostasis of neuronal excitation levels at brain stem and spinal cord. Anti-GlyR autoantibodies are associated with progressive encephalomyelitis with rigidity and myoclonus or stiff person syndrome. These neuromotor disorders are characterized by exaggerated startle, muscle stiffness, and painful spasms, leading to immobility and fatal outcome in some cases. It was hypothesized that imbalance of motoneuronal inhibition by functional impairment of GlyR and receptor internalization are direct consequences of antibody-antigen interference. Here, serum samples of four patients were tested for anti-GlyR autoantibodies and were used for the analysis of the functional impact on the electrophysiological properties of recombinant GlyRs, transiently expressed in HEK293 cells. Furthermore, the recognition pattern of anti- GlyR autoantibodies to human, zebrafish and chimeric GlyRα1 located the epitope to the far N-terminal region. The pathogenicity of anti-GlyR autoantibodies and thereby the autoimmunologic etiology of the disease was confirmed by passive transfer of patient serum to zebrafish (Danio rerio) larvae, that yielded an abnormal escape response – a brain stem reflex that corresponds to the exaggerated startle of afflicted patients. The phenotype was accompanied by profound reduction of GlyR clusters in spinal cord cryosections of treated zebrafish larvae. Together, these novel insights into the pathogenicity of GlyR autoantibodies confirm the concept of a novel neurologic autoimmune disease and might contribute to the development of innovative therapeutic strategies.
Ziel der vorliegenden Arbeit war es, eine psychometrische Analyse des von W. Briegel entwickelten Fragebogens durchzuführen , der die Zufriedenheit mit dem Aufklärungsgespräch über eine medikamentöse Therapie multiperspektivisch aus Sicht der Beteiligten erfassen soll.
An einer Stichprobe von 63 gültigen Fällen erfolgte eine Evaluation des Fragebogens im Sinne einer Itemanalyse, sowie die Überprüfung auf Validität und Reliabilität. Zudem wurde untersucht ob im Allgemeinen Zufriedenheit angegeben wurden und ob Einflussfaktoren auf das Antwortverhalten und die Zufriedenheit festgestellt werden konnten
Insgesamt wurden die Fragen von allen Beteiligten im Sinne von Zufriedenheit beantwortet. Die Itemanalyse erbrachte hohe Schwierigkeitsindices, hohe Trennschärfewerte und geringe Varianzen. Des Weiteren zeigte der Fragebogen gute Werte für die Interne Konsistenz und für die Prüfung auf Kriteriumsvalidität.
Aufgrund der sich ergebenden Hinweise, dass die hohen Werte der Zufriedenheitsbefragung durch die Konstruktionsweise des Fragebogens zustande gekommen sein könnten, erscheint es sinnvoll, die Ratingskala des Fragebogens zu modifizieren. Dieser so veränderte Fragebogen sollte dann an einer heterogeneren Stichprobe erneut eingesetzt werden, um herauszufinden, ob die Ergebnisse in diesem Kontext dieselbe Tendenz zeigen.
The Johnston's organ (JO) in the insect antenna is a multisensory organ involved in several navigational tasks including wind‐compass orientation, flight control, graviception, and, possibly, magnetoreception. Here we investigate the three dimensional anatomy of the JO and its neuronal projections into the brain of the desert ant Cataglyphis, a marvelous long‐distance navigator. The JO of C. nodus workers consists of 40 scolopidia comprising three sensory neurons each. The numbers of scolopidia slightly vary between different sexes (female/male) and castes (worker/queen). Individual scolopidia attach to the intersegmental membrane between pedicel and flagellum of the antenna and line up in a ring‐like organization. Three JO nerves project along the two antennal nerve branches into the brain. Anterograde double staining of the antennal afferents revealed that JO receptor neurons project to several distinct neuropils in the central brain. The T5 tract projects into the antennal mechanosensory and motor center (AMMC), while the T6 tract bypasses the AMMC via the saddle and forms collaterals terminating in the posterior slope (PS) (T6I), the ventral complex (T6II), and the ventrolateral protocerebrum (T6III). Double labeling of JO and ocellar afferents revealed that input from the JO and visual information from the ocelli converge in tight apposition in the PS. The general JO anatomy and its central projection patterns resemble situations in honeybees and Drosophila. The multisensory nature of the JO together with its projections to multisensory neuropils in the ant brain likely serves synchronization and calibration of different sensory modalities during the ontogeny of navigation in Cataglyphis.
Hintergrund
Die Versorgung von Patellafrakturen ist technisch anspruchsvoll. Auch wenn die radiologischen Ergebnisse zumeist zufriedenstellend sind, deckt sich dies häufig nicht mit der subjektiven Einschätzung der Patienten. Die klassische Versorgung mittels Drahtzuggurtung weist einige Komplikationen auf. Die winkelstabile Plattenosteosynthese hat sich in den letzten Jahren biomechanisch als vorteilhaft erwiesen.
Fragestellung
Von wem werden Patellafrakturen in Deutschland versorgt? Wie sieht der aktuelle Versorgungsstandard aus? Haben sich „moderne“ Osteosyntheseformen durchgesetzt? Was sind die häufigsten Komplikationen?
Material und Methoden
Die Mitglieder der Deutschen Gesellschaft für Orthopädie und Unfallchirurgie sowie der Deutschen Kniegesellschaft wurden aufgefordert, an einer Onlinebefragung teilzunehmen.
Ergebnisse
Insgesamt wurden 511 komplett ausgefüllte Fragebogen ausgewertet. Die Befragten sind zum größten Teil auf Unfallchirurgie spezialisiert (51,5 %) und verfügen über langjährige Berufserfahrung in Traumazentren. Die Hälfte der Operateure versorgt ≤5 Patellafrakturen jährlich. In knapp 40 % der Fälle wird die präoperative Bildgebung um eine Computertomographie ergänzt. Die klassische Zuggurtung ist noch die bevorzugte Osteosyntheseform bei allen Frakturtypen (Querfraktur 52 %, Mehrfragmentfrakturen 40 %). Bei Mehrfragmentfrakturen entscheiden sich 30 % der Operateure für eine winkelstabile Plattenosteosynthese. Bei Beteiligung des kaudalen Pols dient als zusätzliche Sicherung die McLaughlin-Schlinge (60 %).
Diskussion
Der Versorgungsstandard von Patellafrakturen in Deutschland entspricht weitgehend der aktualisierten S2e-Leitlinie. Nach wie vor wird die klassische Zuggurtungsosteosynthese als Verfahren der Wahl genutzt. Weitere klinische (Langzeit‑)Studien werden benötigt, um die Vorteile der winkelstabilen Plattenosteosynthese zu verifizieren.
In several countries, a decline in mortality, case-fatality and recurrence rates of stroke was observed. However, studies investigating sex-specific and subtype-specific (pathological and etiological) time trends in stroke mortality, case-fatality and recurrence rates are scarce, especially in Germany. The decline in ischemic stroke mortality and case-fatality might be associated with the high quality of acute care of ischemic stroke, but the exact determinants of early outcome remains unknown for Germany.
Therefore, as first step of this thesis, we investigated the time trends of subtype- and sex-specific age- standardized stroke mortality rates in Germany from 1998 to 2015, by applying joinpoint regression on official causes of death statistics, provided by the Federal Statistical Office. Furthermore, a regional comparison of the time trends in stroke mortality between East and West was conducted. In the second step, time trends in case-fatality and stroke recurrence rates were analyzed using data from a population- based stroke register in Germany between 1996 and 2015. The analysis was stratified by sex and etiological subtype of ischemic stroke. In the third step, quality of stroke care and the association between adherence to measures of quality of acute ischemic stroke care and in-hospital mortality was estimated based on data from nine regional hospital-based stroke registers in Germany from the years 2015 and 2016.
We showed that in Germany, age-standardized stroke mortality declined by over 50% from 1998 to 2015 both, in women and men. Stratified by the pathological subtypes of stroke, the decrease in mortality was larger in ischemic stroke compared to hemorrhagic stroke. Different patterns in the time trends of stroke were observed for stroke subtypes, regions in Germany (former Eastern part of Germany (EG), former Western part of Germany (WG)) and sex, but in all strata a decline was found. By applying joinpoint regression, the number of changes in time trend differed between the regions and up to three changes in the trend in ischemic stroke mortality were detected. Trends in hemorrhagic stroke were in parallel between the regions with up to one change (in women) in joinpoint regression. Comparing the regions, stroke mortality was higher in EG compared to WG throughout the whole observed time period, however the differences between the regions started to diminish from 2007 onwards.
Further it was found that, based on the population-based Erlangen Stroke Project (ESPro), case-fatality and recurrence rates in ischemic stroke patients are still high in Germany. 46% died and 20% got a recurrent stroke within the first five years after stroke. Case-fatality rates declined statistically significant from 1996 to 2015 across all ischemic stroke patients and all etiological subtypes of ischemic stroke. Based on Cox regression no statistically significant decrease in stroke recurrence was observed.
Based on the pooled data of nine regional hospital-based stroke registers from the years 2015 and 2016 covering about 80% of all hospitalized stroke patients in Germany, a high quality of care of acute ischemic stroke patients, measured via 11 evidence-based quality indicators (QI) of process of care, was observed. Across all registers, most QI reached the predefined target values for good quality of stroke care. 9 out of 11 QI showed a significant association with 7-day in-hospital mortality. An inverse linear association between overall adherence to QI and 7-day in-hospital mortality was observed.
In conclusion, stroke mortality and case-fatality showed a favorable development over time in Germany, which might partly be due to improvements in acute treatment. This is supported by the association between overall adherence to quality of care and in-hospital mortality. However, there might be room for improvements in long-term secondary prevention, as no clear reduction in recurrence rates was observed.
Validierung eines klinischen Data Warehouses: Einsatz und Möglichkeiten in der Viszeralchirurgie

Einleitung: In Zeiten des digitalen Fortschritts und wachsender Speicherkapazitäten wird es möglich, immer größere Datenmengen zu verarbeiten. Gleichzeitig besteht der Wunsch, aus diesen Daten neue Informationen im Sinne des „Information retrieval“ zu gewinnen. PaDaWaN ist ein parametrisierbares Data Warehouse Framework zur effizienten Abfrage und Auswertung homogener und heterogener Datenbestände, das 2011 an der Universität Würzburg entwickelt wurde.
Methoden: Zur Validierung des Data Warehouses in der Viszeralchirurgie wurden die automatisiert generierten Daten aus PaDaWaN mit den manuell erhobenen Registerdaten des EuraHS Registers verglichen. Eingeschlossen wurden Patienten mit der Diagnose einer inzisionalen oder primär ventralen Hernie (n=510). Hierfür wurden Informationen zu Diagnosen, Operationen und die intraoperativ verwendeten Materialien aus strukturierten und unstrukturierten Datenquellen des CIS ausgelesen. Das Maß der Übereinstimmung wurde mittels Cohens Kappa-Koeffizienten berechnet (IBM SPSS Statistics 24).
Ergebnisse: Im Rahmen der Studie konnten Diskrepanzen zwischen strukturierten Datenquellen (ICD-10 Codes, OPS Codes) und unstrukturierten Datenquallen (Arztbriefe, Operationsberichte) aufgedeckt werden. Unstimmigkeiten in der ICD-10 Klassifikation für primär ventrale und inzisionale Hernien führten zu einer deutlichen Unterschätzung der inzisionalen umbilikalen Hernien. Sehr gute Übereinstimmungen wurden in den Kategorien Netzimplantation in IPOM-Technik, Underlay- und Sublay-Position erreicht. Faktoren, die die Konkordanz der Datensätze beeinflussten, waren: Erfassung von Vordiagnosen, Voroperationen, mangelndes Erkennen von Negierungen und die Verwendung mehrerer Netze während einer Operation. Klassifikationen wie die "Dietz-Klassifikation" konnten automatisch erkannt und in ihre Bestandteile zerlegt werden.
Fazit: Durch die Etablierung von Data Warehousing als Plattform für die klinische Forschung können Daten in Zukunft schneller strukturiert und generiert werden. Durch die dynamische tägliche automatisierte Datenaktualisierung kann das klinische Personal Behandlungskonzepte und Ergebnisse schneller validieren und bewerten. Darüber hinaus können Empfehlungen für zukünftige medizinische Dokumentation gegeben werden, um die Informationsextraktion von PaDaWaN zu verbessern. Die Ergebnisse dieser Studie zeigen deutliche Diskrepanzen zwischen strukturierten und unstrukturierten Datenquellen. Vorhandene Register und Daten des CIS können zukünftig im Sinne einer internen Validierung verifiziert und damit manuelle Dokumentationsfehler nachhaltig aufgedeckt werden.
Hintergrund: Die konventionelle Trabekulektomie (TET) stellt trotz einem eher ungünstigen Komplikationsprofil weiterhin den Goldstandard der operativen Glaukomtherapie dar, da sie eine effektivere langfristige Drucksenkung als neuere Operationsverfahren aufweist.
Fragestellung: Vergleich der Erfolgsquote und des Risikoprofils der TET mit einer durch Schaffen einer zweiten Filtrationsebene sowie Vermeiden einer peripheren Iridektomie modifizierten Trabekulektomie („Filtrierende Trabekulotomie“ (FTO)).
Material und Methode: Retrospektiv durchgeführter intraindividueller Vergleich über 36 Monate an 20 Patienten mit Offenwinkelglaukom nach TET an einem Auge und FTO am anderen Auge an der Augenklinik der Universität Würzburg. Primärer Endpunkt war das Erreichen des absoluten/Teilerfolgs (IOD ≤18 mmHg und ≥ 20 % Druckreduktion ohne/mit Medikation). Als sekundäre Endpunkte wurden das Auftreten von Komplikationen sowie der Verlauf von Augeninnendruck, Visus und lokaler Pharmakotherapie analysiert.
Ergebnisse: Beide Operationsverfahren führten zu einer signifikanten Reduktion des Augeninnendrucks zu jedem postoperativen Zeitpunkt. Nach 36 Monaten zeigte sich ein absoluter Erfolg bei 50 % der Augen in der TET-Gruppe und 20 % der FTO-Gruppe sowie ein Teilerfolg bei 71,4 % gegenüber 33,3 %. Zu Komplikationen kam es mit Ausnahme des häufigeren Auftretens eines Hyphämas in der FTO-Gruppe in vergleichbarem Ausmaß in beiden Gruppen. Der Visus und postoperative Medikamentenscore unterschieden sich nicht signifikant voneinander.
Schlussfolgerung: Die TET war der FTO in Hinblick auf Erfolg und Komplikationsrisiko überlegen. Die möglichen Vorteile der Operation durch die genannten Modifikationen konnten nicht bestätigt werden.
Neuropathies are a group of potentially treatable diseases with an often disabling and restricting course. Amyotrophic lateral sclerosis (ALS) is a lethal disease without causal treatment possibilities. The objective of this study was to examine the diagnostic utility of HRUS for the differentiation of subtypes of axonal and demyelinating neuropathies and to investigate its utility for the sonological differentiation of ALS.
The hypothetical statement that neuropathy causes enlargement of peripheral nerves compared to healthy controls proved to be right, but the adjunctive assumption that ALS does not cause enlargement of peripheral nerves proved to be wrong – in patients with ALS slight enlargement of peripheral nerves was visible as well. The statement that nerve enlargement can be detected by measurement of the cross-sectional area (CSA) and the longitudinal diameter (LD) with comparable results proved to be right, but the enlargement was slightly less present by measurement of the LD. The statement that axonal and demyelinating neuropathies show distinct patterns of nerve enlargement must be answered differentiated: The comparison between axonal and demyelinating neuropathies showed a stronger nerve enlargement in patients with demyelinating neuropathies than in patients with axonal neuropathies at proximal nerve segments of upper extremities. In the comparison of diagnose-defined subgroups of inflammatory demyelinating neuropathies a respective specific pattern of nerve enlargement was visible. However, remarkable in this context was the strong nerve enlargement found in patients with NSVN, which is classified as an axonal neuropathy. Stratification for specific findings in nerve biopsy did not lead to constructive differences in comparison between the different groups.
To sum up, HRUS showed to provide a useful contribution in the diagnostic process of neuropathies and ALS but needs to be integrated in a multimodal diagnostic approach.
The Myb-MuvB (MMB) complex plays an essential role in the time-dependent transcriptional activation of mitotic genes. Recently, our laboratory identified a novel crosstalk between the MMB-complex and YAP, the transcriptional coactivator of the Hippo pathway, to coregulate a subset of mitotic genes (Pattschull et al., 2019). Several genetic studies have shown that the Hippo-YAP pathway is essential to drive cardiomyocyte proliferation during cardiac development (von Gise et al., 2012; Heallen et al., 2011; Xin et al., 2011). However, the exact mechanisms of how YAP activates proliferation of cardiomyocytes is not known. This doctoral thesis addresses the physiological role of the MMB-Hippo crosstalk within the heart and characterizes the YAP-B-MYB interaction with the overall aim to identify a potent inhibitor of YAP.
The results reported in this thesis indicate that complete loss of the MMB scaffold protein LIN9 in heart progenitor cells results in thinning of ventricular walls, reduced cardiomyocyte proliferation and early embryonic lethality. Moreover, genetic experiments using mice deficient in SAV1, a core component of the Hippo pathway, and LIN9-deficient mice revealed that the correct function of the MMB complex is critical for proliferation of cardiomyocytes due to Hippo-deficiency. Whole genome transcriptome profiling as well as genome wide binding studies identified a subset of Hippo-regulated cell cycle genes as direct targets of MMB. By proximity ligation assay (PLA), YAP and B-MYB were discovered to interact in embryonal cardiomyocytes. Biochemical approaches, such as co-immunoprecipitation assays, GST-pulldown assays, and µSPOT-based peptide arrays were employed to characterize the YAP-B-MYB interaction. Here, a PY motif within the N-terminus of B-MYB was found to directly interact with the YAP WW-domains. Consequently, the YAP WW-domains were important for the ability of YAP to drive proliferation in cardiomyocytes and to activate MMB target genes in differentiated C2C12 cells. The biochemical information obtained from the interaction studies was utilized to develop a novel competitive inhibitor of YAP called MY-COMP (Myb-YAP competition). In MY-COMP, the protein fragment of B-MYB containing the YAP binding domain is fused to a nuclear localization signal. Co-immunoprecipitation studies as well as PLA revealed that the YAP-B-MYB interaction is robustly blocked by expression of MY-COMP. Adenoviral overexpression of MY-COMP in embryonal cardiomyocytes suppressed entry into mitosis and blocked the pro-proliferative function of YAP. Strikingly, characterization of the cellular phenotype showed that ectopic expression of MY-COMP led to growth defects, nuclear abnormalities and polyploidization in HeLa cells.
Taken together, the results of this thesis reveal the mechanism of the crosstalk between the Hippo signaling pathway and the MMB complex in the heart and form the basis for interference with the oncogenic activity of the Hippo coactivator YAP.
Biofabrication, including printing technologies, has emerged as a powerful approach to the design of disease models, such as in cancer research. In breast cancer, adipose tissue has been acknowledged as an important part of the tumor microenvironment favoring tumor progression. Therefore, in this study, a 3D-printed breast cancer model for facilitating investigations into cancer cell-adipocyte interaction was developed. First, we focused on the printability of human adipose-derived stromal cell (ASC) spheroids in an extrusion-based bioprinting setup and the adipogenic differentiation within printed spheroids into adipose microtissues. The printing process was optimized in terms of spheroid viability and homogeneous spheroid distribution in a hyaluronic acid-based bioink. Adipogenic differentiation after printing was demonstrated by lipid accumulation, expression of adipogenic marker genes, and an adipogenic ECM profile. Subsequently, a breast cancer cell (MDA-MB-231) compartment was printed onto the adipose tissue constructs. After nine days of co-culture, we observed a cancer cell-induced reduction of the lipid content and a remodeling of the ECM within the adipose tissues, with increased fibronectin, collagen I and collagen VI expression. Together, our data demonstrate that 3D-printed breast cancer-adipose tissue models can recapitulate important aspects of the complex cell–cell and cell–matrix interplay within the tumor-stroma microenvironment
Megakaryocytes (MKs) release platelets into the lumen of bone marrow (BM) sinusoids while remaining to reside within the BM. The morphogenetic events of this complex process are still not fully understood. We combined confocal laser scanning microscopy with transmission and serial block-face scanning electron microscopy followed by 3D-reconstruction on mouse BM tissue sections. These analyses revealed that MKs in close vicinity to BM sinusoid (BMS) wall first induce the lateral retraction of CXCL12-abundant reticular (CAR) cells (CAR), followed by basal lamina (BL) degradation enabling direct MK-sinusoidal endothelial cells (SECs) interaction. Subsequently, an endothelial engulfment starts that contains a large MK protrusion. Then, MK protrusions penetrate the SEC, transmigrate into the BMS lumen and form proplatelets that are in direct contact to the SEC surface. Furthermore, such processes are induced on several sites, as observed by 3D reconstructions. Our data demonstrate that MKs in interaction with CAR-cells actively induce BMS wall alterations, including CAR-cell retraction, BL degradation, and SEC engulfment containing a large MK protrusion. This results in SEC penetration enabling the migration of MK protrusion into the BMS lumen where proplatelets that are adherent to the luminal SEC surface are formed and contribute to platelet release into the blood circulation.
Atherosclerosis is an inflammatory disease of large and medium-sized arteries, characterized by the growth of atherosclerotic lesions (plaques). These plaques often develop at inner curvatures of arteries, branchpoints, and bifurcations, where the endothelial wall shear stress is low and oscillatory. In conjunction with other processes such as lipid deposition, biomechanical factors lead to local vascular inflammation and plaque growth. There is also evidence that low and oscillatory shear stress contribute to arterial remodeling, entailing a loss in arterial elasticity and, therefore, an increased pulse-wave velocity. Although altered shear stress profiles, elasticity and inflammation are closely intertwined and critical for plaque growth, preclinical and clinical investigations for atherosclerosis mostly focus on the investigation of one of these parameters only due to the experimental limitations. However, cardiovascular magnetic resonance imaging (MRI) has been demonstrated to be a potent tool which can be used to provide insights into a large range of biological parameters in one experimental session. It enables the evaluation of the dynamic process of atherosclerotic lesion formation without the need for harmful radiation. Flow-sensitive MRI provides the assessment of hemodynamic parameters such as wall shear stress and pulse wave velocity which may replace invasive and radiation-based techniques for imaging of the vascular
function and the characterization of early plaque development. In combination with inflammation imaging, the analyses and correlations of these parameters could not only significantly advance basic preclinical investigations of atherosclerotic lesion formation and progression, but also the diagnostic clinical evaluation for early identification of high-risk plaques, which are prone to rupture. In this review, we summarize the key applications of magnetic resonance imaging for the evaluation of plaque characteristics through flow sensitive and morphological measurements. The simultaneous measurements of functional and structural parameters will further preclinical research on atherosclerosis and has the potential to fundamentally improve the detection of inflammation and vulnerable plaques in patients.
