Background
In individuals suffering from a rare disease the diagnostic process and the confirmation of a final diagnosis often extends over many years. Factors contributing to delayed diagnosis include health care professionals' limited knowledge of rare diseases and frequent (co-)occurrence of mental disorders that may complicate and delay the diagnostic process. The ZSE-DUO study aims to assess the benefits of a combination of a physician focusing on somatic aspects with a mental health expert working side by side as a tandem in the diagnostic process.
Study design
This multi-center, prospective controlled study has a two-phase cohort design.
Methods
Two cohorts of 682 patients each are sequentially recruited from 11 university-based German Centers for Rare Diseases (CRD): the standard care cohort (control, somatic expertise only) and the innovative care cohort (experimental, combined somatic and mental health expertise). Individuals aged 12 years and older presenting with symptoms and signs which are not explained by current diagnoses will be included. Data will be collected prior to the first visit to the CRD’s outpatient clinic (T0), at the first visit (T1) and 12 months thereafter (T2).
Outcomes
Primary outcome is the percentage of patients with one or more confirmed diagnoses covering the symptomatic spectrum presented. Sample size is calculated to detect a 10 percent increase from 30% in standard care to 40% in the innovative dual expert cohort. Secondary outcomes are (a) time to diagnosis/diagnoses explaining the symptomatology; (b) proportion of patients successfully referred from CRD to standard care; (c) costs of diagnosis including incremental cost effectiveness ratios; (d) predictive value of screening instruments administered at T0 to identify patients with mental disorders; (e) patients’ quality of life and evaluation of care; and f) physicians’ satisfaction with the innovative care approach.
Conclusions
This is the first multi-center study to investigate the effects of a mental health specialist working in tandem with a somatic expert physician in CRDs. If this innovative approach proves successful, it will be made available on a larger scale nationally and promoted internationally. In the best case, ZSE-DUO can significantly shorten the time to diagnosis for a suspected rare disease.
Indirekte Charakterisierung trägt zu einem großen Teil zur Gesamtcharakteristik von Figuren bei, ist bisher aber dennoch vergleichsweise wenig erforscht. Diese Arbeit nähert sich dem Phänomen der indirekten Figurencharakterisierung durch eine eigene Explikation theoretisch an, nennt verschiedene Repräsentationsformen von indirekter Charakterisierung und führt die Theorie dann in Bezug auf Redewiedergabe anhand zahlreicher Beispiele aus. Der Untersuchung wird ein Korpus realistischer Erzähltexte zugrunde gelegt und darin manuell Formen der Redewiedergabe annotiert, um in einem späteren Schritt computergestützt einige Experimente mit den Annotationen durchzuführen. In ihrem Aufbau entspricht diese Arbeit dadurch einem Mixed-Methods-Design von qualitativer und quantitativer Literaturwissenschaft.
Current data show that resilience is an important factor in cancer patients’ well-being. We aim to explore the resilience of patients with lower grade glioma (LGG) and the potentially influencing factors. We performed a cross-sectional assessment of adult patients with LGG who were enrolled in the LoG-Glio registry. By phone interview, we administered the following measures: Resilience Scale (RS-13), distress thermometer, Montreal Cognitive Assessment Test for visually impaired patients (MoCA-Blind), internalized stigmatization by brain tumor (ISBI), Eastern Cooperative Oncological Group performance status (ECOG), patients’ perspective questionnaire (PPQ) and typical clinical parameters. We calculated correlations and multivariate regression models. Of 74 patients who were assessed, 38% of those showed a low level of resilience. Our results revealed significant correlations of resilience with distress (p < 0.001, −0.49), MOCA (p = 0.003, 0.342), ECOG (p < 0.001, −0.602), stigmatization (p < 0.001, −0.558), pain (p < 0.001, −0.524), and occupation (p = 0.007, 0.329). In multivariate analyses, resilience was negatively associated with elevated ECOG (p = 0.020, β = −0.383) and stigmatization levels (p = 0.008, β = −0.350). Occupation showed a tendency towards a significant association with resilience (p = 0.088, β = −0.254). Overall, low resilience affected more than one third of our cohort. Low functional status is a specific risk factor for low resilience. The relevant influence of stigmatization on resilience is a novel finding for patients suffering from a glioma and should be routinely identified and targeted in clinical routine.
Brain metastases are the most severe tumorous spread during breast cancer disease. They are associated with a limited quality of life and a very poor overall survival. A subtype of extracellular vesicles, exosomes, are sequestered by all kinds of cells, including tumor cells, and play a role in cell-cell communication. Exosomes contain, among others, microRNAs (miRs). Exosomes can be taken up by other cells in the body, and their active molecules can affect the cellular process in target cells. Tumor-secreted exosomes can affect the integrity of the blood-brain barrier (BBB) and have an impact on brain metastases forming. Serum samples from healthy donors, breast cancer patients with primary tumors, or with brain, bone, or visceral metastases were used to isolate exosomes and exosomal miRs. Exosomes expressed exosomal markers CD63 and CD9, and their amount did not vary significantly between groups, as shown by Western blot and ELISA. The selected 48 miRs were detected using real-time PCR. Area under the receiver-operating characteristic curve (AUC) was used to evaluate the diagnostic accuracy. We identified two miRs with the potential to serve as prognostic markers for brain metastases. Hsa-miR-576-3p was significantly upregulated, and hsa-miR-130a-3p was significantly downregulated in exosomes from breast cancer patients with cerebral metastases with AUC: 0.705 and 0.699, respectively. Furthermore, correlation of miR levels with tumor markers revealed that hsa-miR-340-5p levels were significantly correlated with the percentage of Ki67-positive tumor cells, while hsa-miR-342-3p levels were inversely correlated with tumor staging. Analysis of the expression levels of miRs in serum exosomes from breast cancer patients has the potential to identify new, non-invasive, blood-borne prognostic molecular markers to predict the potential for brain metastasis in breast cancer. Additional functional analyzes and careful validation of the identified markers are required before their potential future diagnostic use.
Objectives
Magnesium phosphate-based cements begin to catch more attention as bone substitute materials and especially as alternatives for the more commonly used calcium phosphates. In bone substitutes for augmentation purposes, atraumatic materials with good biocompatibility and resorbability are favorable. In the current study, we describe the in vivo testing of novel bone augmentation materials in form of spherical granules based on a calcium-doped magnesium phosphate (CaMgP) cement.
Materials and Methods
Granules with diameters between 500 and 710 μm were fabricated via the emulsification of CaMgP cement pastes in a lipophilic liquid. As basic material, two different CaMgP formulations were used. The obtained granules were implanted into drill hole defects at the distal femoral condyle of 27 New Zealand white rabbits for 6 and 12 weeks. After explantation, the femora were examined via X-ray diffraction analysis, histological staining, radiological examination, and EDX measurement.
Results
Both granule types display excellent biocompatibility without any signs of inflammation and allow for proper bone healing without the interposition of connective tissue. CaMgP granules show a fast and continuous degradation and enable fully adequate bone regeneration.
Conclusions
Due to their biocompatibility, their degradation behavior, and their completely spherical morphology, these CaMgP granules present a promising bone substitute material for bone augmentation procedures, especially in sensitive areas.
Clinical Relevance
The mostly insufficient local bone supply after tooth extractions complicates prosthetic dental restoration or makes it even impossible. Therefore, bone augmentation procedures are oftentimes inevitable. Spherical CaMgP granules may represent a valuable bone replacement material in many situations.
Digital platforms, such as Amazon, represent the major beneficiaries of the Covid‐19 crisis. This study examines the role of digital platforms and their engagement in digitalisation initiatives targeting (small) brick‐and‐mortar retailers in Germany, thereby contributing to a better understanding of how digital platforms augment, substitute or reorganise physical retail spaces. This study applies a mixed‐method approach based on qualitative interviews, participant observation as well as media analysis. First, the study illustrates the controversial role of digital platforms by positioning themselves as supporting partners of the (offline) retailers, while simultaneously shifting power towards the platforms themselves. Second, digital platforms have established themselves not only as infrastructure providers but also as actors within these infrastructures, framing digital as well as physical retail spaces, inter alia due to their role as publicly legitimised retail advisers. Third, while institutions want to help retailers to survive, they simultaneously enhance retailers' dependency on digital platforms.
Rare variants in at least 10 genes, including BRCA1, BRCA2, PALB2, ATM, and CHEK2, are associated with increased risk of breast cancer; however, these variants, in combination with common variants identified through genome-wide association studies, explain only a fraction of the familial aggregation of the disease. To identify further susceptibility genes, we performed a two-stage whole-exome sequencing study. In the discovery stage, samples from 1528 breast cancer cases enriched for breast cancer susceptibility and 3733 geographically matched unaffected controls were sequenced. Using five different filtering and gene prioritization strategies, 198 genes were selected for further validation. These genes, and a panel of 32 known or suspected breast cancer susceptibility genes, were assessed in a validation set of 6211 cases and 6019 controls for their association with risk of breast cancer overall, and by estrogen receptor (ER) disease subtypes, using gene burden tests applied to loss-of-function and rare missense variants. Twenty genes showed nominal evidence of association (p-value < 0.05) with either overall or subtype-specific breast cancer. Our study had the statistical power to detect susceptibility genes with effect sizes similar to ATM, CHEK2, and PALB2, however, it was underpowered to identify genes in which susceptibility variants are rarer or confer smaller effect sizes. Larger sample sizes would be required in order to identify such genes.
In Ratten und Mäusen aktiviert der superagonistische anti-CD28 monoklonale Antikörper (CD28SA) vorzugsweise regulatorische T-Zellen. In niedriger Dosierung führt CD28SA zu einer fast ausschließlichen Aktivierung von regulatorischen T-Zellen (Tregs). Diese Beobachtung konnte inzwischen auch für menschliche Zellen in Zellkultur bestätigt werden.
In gesunden und freiwilligen Testpersonen deutet die Zytokin-Antwort nach Applikationen von niedrigen CD28SA-Dosen darauf hin, dass sich diese Beobachtung auch in-vivo bewahrheitet. Eine Gabe von CD28SA in niedriger Dosierung, die zu einer exklusiven Aktivierung von regulatorischen T-Zellen führt, könnte somit in der Behandlung von Autoimmunkrankheiten oder von entzündlichen Erkrankungen eingesetzt werden.
Eine mechanistische Erklärung für dieses Phänomen blieb lange Zeit unklar. Die CD28SA-vermittelte T-Zell-Aktivierung ist abhängig von der Verstärkung von basalen tonischen Signalen, die T-Zellen über ihren T-Zell-Rezeptor erhalten. Diese Tatsache führte zu der Hypothese, dass die schwachen, tonischen Signale, die konventionelle CD4+ T-Zellen in Abwesenheit ihrer spezifischen Antigene über den T-Zell-Rezeptor erhalten, ein stärkeres CD28 Signal für ihre Aktivierung benötigen als die selbstreaktiven regulatorischen T-Zellen, die ein stärkeres Selbstpeptid-TCR Signal erhalten.
In dieser Arbeit konnte gezeigt werden, dass die Blockade von MHC-Klasse-II-Molekülen in Mäusen, in-vitro und in-vivo, den Vorteil der regulatorischen T-Zellen gegenüber den konventionellen T-Zellen bezüglich der Antwort auf niedrige CD28SA Dosierungen, aufhebt.
