Im metastasierten Melanom sind bei Vorhandensein einer BRAF V600 Mutation zielgerichtete Therapien mit BRAF+MEK-Inhibitoren sowie Immuntherapien (ICB), die Immuncheckpoints wie PD-1 blockieren, zugelassen. Aktuell gibt es keine evidenzbasierte Empfehlung welche Therapie in der Erstlinie im BRAF V600 mutierten Melanom eingesetzt werden sollte. Bis jetzt wurde der Stellenwert PD-1 basierter Immuncheckpoint Blockade in der Zweitlinie nach Progress unter BRAF+MEK-Inhibition nicht beschrieben. Es ist auch unklar, ob die Kombinations-ICB (PD-1 plus CTLA-4 Blockade) mit einer Verbesserung des Ansprechens und Überlebens gegenüber einer PD-1 Monotherapie assoziiert ist, wie für das therapie-naive Melanom beschrieben. Wir haben eine retrospektive, multizentrische Studie durchgeführt um die Wirksamkeit von PD-1 basierten Immuntherapien nach Progress unter zielgerichteter Therapie zu explorieren. In unserer Untersuchung zeigten PD-1 Monotherapie und die kombinierte PD-1 plus CTLA-4 Blockade eine ähnliche Wirksamkeit in Patienten mit BRAFi+MEKi-Resistenz. Die Kombinationstherapie war dagegen mit einem deutlich höheren Risiko für schwerwiegende immunvermittelte Nebenwirkungen im Vergleich zu PD-1 Monotherapie assoziiert. Unsere Daten indizieren, dass eine PD-1 Blockade einer Kombinations-ICB in der Zweitlinie nach Progress unter zielgerichteter Therapie im fortgeschrittenen BRAF V600 mutierten Melanom vorzuziehen ist.
Die vorliegende retrospektive Untersuchung fasst Ergebnisse bei 69 Säuglingen zusammen, die in den Jahren 2010 bis 2014 aufgrund infantiler Hämangiome mit eingetretenen oder drohenden Komplikationen am Universitätsklinikum Würzburg mit dem Betablocker Propranolol oral behandelt wurden. Ergänzend wurden von 45 der 69 Elternpaare nach Therapieende Fragebögen zur subjektiven Einschätzung der Behandlung ihrer Kinder ausgefüllt. Die Ergebnisse dieser Studie zeigen in Zusammenschau mit zahlreichen Publikationen zu diesem Thema, dass Propranolol bei proliferierenden Säuglingshämangiomen eine sichere und hocheffektive Therapie bei gleichzeitig überschaubaren und nur in Einzelfällen gravierenden Nebenwirkungen hat.
Infektionen mit der Krätzemilbe Sarcoptes scabiei varietas hominis können in Gemeinschaftseinrichtungen wie Krankenhäusern, Altenpflege- oder Behindertenheimen weit reichende Ausbrüche hervorrufen. In einer großen Werkstatt für behinderte Menschen und den angegliederten Wohnheimen kam es ab August 2003 zu einem weit reichenden Skabiesausbruch, welcher sich über 10 Monate hinzog. Erst nachdem Vertreter der Werkstatt und Wohnheime und Ärzte der Universitäts-Hautklinik gemeinsam ein Behandlungskonzept erstellt hatten, konnte die Skabies erfolgreich eingedämmt werden. Aufgrund der dabei gemachten Erfahrungen sowie in der Fachliteratur publizierter Skabiesausbrüche wurde ein umfangreiches Konzept zum Vorgehen bei Skabiesausbrüchen in Gemeinschaftseinrichtungen erstellt.
Die vorliegende Arbeit hatte das Ziel, erstmals unterschiedliche automatisierte Bildanalysesysteme in der Auswertung von Elispot-Proben miteinander zu vergleichen. Neben dem Vergleich der Anzahl der gemessenen Spots, sollten die Zeit für die Messungen, die Genauigkeit, Richtigkeit und Präzision untersucht werden. Weiterhin sollten die Zuverlässigkeit der Messungen sowie die Einflussgrößen der einzelnen Bildanalysesysteme geprüft werden. Es wurden Elispotproben von verschiedenen Spezies (Maus und Human) und von verschiedenen unabhängigen Arbeitsgruppen verwendet. Die verglichenen Bildanalysesysteme bestanden aus den aufeinander abgestimmten Komponenten Mikroskop, Farbkamera, Motortisch mit Steuerung, Analysesoftware und Rechner. Sie unterschieden sich in ihren Mikroskopen, in der Anzahl der pro Well aufgenommenen Bilder und in der damit erreichten Bildpunktauflösung. Beim KS Elispot wurden im Auflichtmikroskop pro Well 12 Bilder über eine Farbkamera aufgenommen, die Bildpunktauflösung für ein 760x580 Pixel Bild eines Wells betrug 2.6µm. Beim KS Elispot compact 0.65-Zoom-Einstellung wurde im Stereomikroskop pro Well 1 Bild über eine Farbkamera aufgenommen und eine Bildpunktauflösung von 12µm erreicht. Beim KS Elispot 1.25-Zoom-Einstellung wurden im Stereomikroskop 4 Bilder pro Well über eine Farbkamera erstellt, die Bildpunktauflösung betrug 6µm. Im Bezug auf den Faktor Zeit war das KS Elispot compact dem KS Elispot deutlich überlegen. Bei Verwendung eines Bildes pro Well bzw. 4 Bildern pro Well wertete das KS Elispot compact eine komplette 96-Well-Mikrotiterplatte bis zu 6 Mal schneller bzw. mindestens doppelt so schnell aus wie das KS Elispot. Die hohe Auflösung des KS Elispot resultiert in einer langen Auswertungszeit der einzelnen Platten. Werden große Mengen von Elispot-Proben ausgewertet, so bietet das KS Elispot compact aufgrund seiner kürzeren Messzeiten eine deutliche Ersparnis an Zeit und damit gegenüber dem KS Elispot unter diesem Gesichtspunkt einen entscheidenden Vorteil. Die Variabilität der Messungen lag bei allen Systemen niedrig, ohne nennenswerte Unterschiede zwischen den Systemen. Die höchste Zuverlässigkeit bei der Spoterkennung konnte für das System KS Elispot nachgewiesen werden. Es erkannte nahezu alle echten Spots und mehr echte Spots als das KS Elispot compact. Das KS Elispot wies im Gegensatz zum KS Elispot compact keine falsch-positiven Spots auf. Bei Verwendung von 4 Bildern pro Well arbeitete das KS Elispot compact zuverlässiger als bei Erstellung von nur einer Aufnahme pro Well: der Anteil an falsch-positiven Spots am Ergebnis sank und der Anteil der richtig erkannten Spots stieg. Die Werte lagen jedoch immer noch unterhalb der Ergebnisse, die das KS Elispot erzielt hatte. Das KS Elispot compact erkannte grundsätzlich in denselben Wells weniger Spots als das KS Elispot, das der tatsächlichen Anzahl der Spots am nächsten kam. Bei Verwendung von nur einer Aufnahme pro Well identifizierte das KS Elispot compact deutlich weniger Spots als bei Aufnahme von 4 Bildern pro Well. Die deutlichsten Unterschiede zwischen den beiden Systemen KS Elispot und KS Elispot compact wurden bei der Messung durch das KS Elispot compact mit einem Bild pro Well bei Spotzahlen über 100 bei Mausspots und über 400 bei Humanspots gesehen. Die zwischen dem KS Elispot und dem KS Elispot compact nachgewiesenen Unterschiede waren bei kleinen Spots (bis 100µm) deutlich größer als bei Spots größerer Durchmesser. Der Vergleich der Systeme erbrachte, dass das hochauflösende KS Elispot eine bessere Auswertungsqualität als das KS Elispot compact bietet, insbesondere bei der Auswertung von Elispot-Proben, die sehr viele und zudem sehr kleine Spots enthielten. Beim KS Elispot compact war die Messung mit 4 Bildern pro Well der Auswertung mit nur einem Bild pro Well bezüglich der Zuverlässigkeit klar überlegen. Bei Verwendung des KS Elispot compact sollte deshalb bei sehr kleinen Spots zumindest die mit 4 Bildern pro Well arbeitende Einstellung gewählt werden. Weiterhin ist zu bemerken, dass eine optimale Auswertung von Elispot-Proben durch automatisierte Reader-Systeme maßgeblich durch die Präparation der verwendeten Elispot-Proben und die daraus resultierende Qualität der Spots beeinflusst wird. Zahlreiche Artefakte, eine starke Untergrundfärbung oder nicht deutlich ausgebildete typische Spotmerkmale können die Messergebnisse eines Systems beeinträchtigen. Hierbei wurde das KS Elispot compact System stärker beeinflusst als das KS Elispot System. In dieser Arbeit konnte nachgewiesen werden, dass die Auswertung der Elispot-Proben und damit die gewonnenen Ergebnisse von dem verwendeten, automatisierten Lesesystem abhängen.Die Arbeit unterstreicht den hohen Stellenwert der Standardisierung, Validierung und Optimierung aller Komponenten der Elispot-Methode. Dies ist auch gerade in Bezug auf die Weiterentwicklung dieser Technik und die Eröffnung von weiteren Einsatzspektren unerlässlich.
Das Ziel der vorliegenden Arbeit war die retrospektive Datenerhebung der von Patienten mit Tinea capitis, die zwischen 1990 und 2014 in der dermatologischen Abteilung behandelt bzw. im mykologischen Labor der Universitätsklinik Würzburg diagnostiziert wurden. Zunächst wurden Daten (Geburtsdatum, Alter, Geschlecht, eingesendetes Material, Erreger und eventuelle weitere Pilzerkrankungen) mit Hilfe der Laborbücher ab dem Jahr 1990 gewonnen. Insgesamt wurden 150 diagnostizierte Patientenfälle erfasst. Zusätzlich wurden alle aus den Laborbüchern identifizierten Fälle ab dem Jahr 2002 (n=55) mit den vorhandenen, digitalen Karteikarten im SAP abgeglichen und standardisierte Parameter erfasst (Herkunft, Vorerkrankungen, Medikamentenanamnese, Raucheranamnese, Alkoholanamnese, Diagnose, Therapie, Krankheitsverlauf). Die statistische Datenverarbeitung erfolgte mit dem Programm IBM SPSS Statistics 23 für Mac. Zusätzlich wurden die Daten anhand der Zeiträume von 01/1990- 6/2002 und 07/2002- 12/2014 miteinander verglichen.
Der Anteil an Tinea capitis in Bezug zu allen kulturell nachgewiesenen Dermatomykosen wie Tinea pedum et unguium pedum, Tinea corporis, Tinea faceii, Tinea barbae und Tinea manum lag bei lediglich 3,4%.
Die Patienten waren durchschnittlich 12 Jahre alt. Die Altersspanne erstreckte sich zwischen 0 und 78 Jahren. Auffallend ist der deutlich geringere Median von 6 Jahren und der ebenso niedrigere Wert der 75. Perzentile von 10,25 Jahren. Der Durchschnittswert von 12 Jahren ist also ein, durch Patienten mit einem hohen Alter, täuschender Wert. Die Erkrankung dominiert in der Altersgruppe der 0- bis 5-jährigen Kinder, mit einem deutlichen Peak bei den 3-Jährigen. Die zunehmende Betreuung von Kleinkindern in Gemeinschaftseinrichtungen ist als mögliche Infektionsquelle zu diskutieren. Daher sollten allgemein verbindliche Regelungen zur Isolation von Kindern mit einer durch anthropophile Dermatophyten verursachten Tinea capitis erstellt werden. Der Anteil der Erwachsenen (ab 18 Jahre) liegt bei ungewöhnlich hohen 16%, da Tinea capitis üblicherweise als pädiatrische Mykose bekannt ist. Die klinische Manifestation einer Tinea capitis ist oft polymorph und atypisch, so dass auch im adulten Alter bei einer vorhandenen Symptomatik am Kapillitium als Differentialdiagnose eine Dermatophytose in Betracht gezogen und ggf. entsprechende Diagnostik veranlasst werden sollte. Mit dementsprechenden 84% der Patienten unter 18 Jahren hat die Tinea capitis auch in dieser Untersuchung eine bedeutende Präsenz im pädiatrischen Patientengut. Daher sollte bei Veränderungen am Kapillitium eine Tinea capitis als Differentialdiagnose in Betracht gezogen werden. Die Geschlechterverteilung zeigt eine signifikante Tendenz zum männlichen Geschlecht mit 61,3% (n=92). Zwischen 01/1990 und 06/2002 war der bevorzugte Befall männlicher Patienten ausgeprägter als im nachfolgenden Zeitraum. Geschlechtsspezifische Gewohnheiten wie die Ausübung verschiedener Sportarten könnten ursächlich sein. So ist der T. tonsurans, der wegen seiner Übertragungswege auch als „Ringerpilz“ bezeichnet wird, in der Altersgruppe der 11- bis 17-jährigen Patienten am häufigsten nachgewiesene Erreger. Das weibliche Geschlecht war in dieser Altersgruppe deutlich unterrepräsentiert.
Das Erregerspektrum hat sich im zeitlichen Verlauf von 01/1990 bis 12/2014 mit einer zunehmenden Diversität gezeigt. Führender Erreger im gesamten Zeitraum ist der zoophile Microsporum canis (38,7%). Für eine erfolgreiche Therapie hat die interdisziplinäre Zusammenarbeit zwischen Dermatologen und Veterinärmedizinern einen hohen Stellenwert. Insgesamt haben die zoophilen Dermatophyten einen Anteil von 55,3 %. Beachtenswert ist T. tonsurans als zweithäufigster Erreger (24%). Zusammen mit T. rubrum bedingt T. tonsurans den Hauptteil der beträchtlichen Prozentzahl der anthropophilen Dermatophyten einer Tinea capitis (44%). Zur Kontrolle einer anthropophilen Tinea capitis ist bei geringer klinischer Symptomatik eine mykologische Diagnostik aller Familienangehörigen indiziert. Um Reinfektionen zu meiden, sollte die Therapie der erkrankten Familienangehörigen simultan erfolgen. Im Erwachsenenalter trat T. rubrum als häufigster Erreger der Tinea capitis auf. Geophile Erreger sind nur selten Ursache einer Tinea capitis; entsprechend konnte nur ein einziges Mal M. gypseum isoliert werden. Die frühzeitige Diagnose und eine geeignete, „spezies-spezifische“ Therapie hilft Ausbrüche zu vermeiden. Anhand der aktuellen Flüchtlingswelle aus Afrika und Asien nach Europa ist eine epidemiologische Veränderung des Erregerspektrums der Tinea capitis zu erwarten. Ein Screening, auch um andere infektiöse, mykologische Erkrankungen auszuschließen oder ggf. rechtzeitig zu therapieren, ist angeraten, um eine Infektion des Umfeldes zu vermeiden.
In der vorgelegten Promotionsarbeit wurden die typischen bakteriellen MSSA bzw. MRSA Hautinfektionen einer dermatologischen Klinik mit dem Einzugsgebiet Nordbayern auf krankheitsrelevante Faktoren von PVL untersucht.
Interessanterweise fand sich bei der Präsenz von PVL keine Korrelation mit Methicillinresistenz oder Krankheitsschwere. Weder atopische Diathese noch Rauchen oder Körpergewicht scheinen das Auftreten des Pathogenitätsfaktors zu begünstigen. Allerdings traten die PVL positiven S. aureus Hautinfektionen bevorzugt bei jüngeren und weiblichen Patienten auf. Bei den untersuchten Hauterkrankungen zeigten S. aureus Stämme eine ausgeprägte Vielfalt. Es konnte kein spezieller epidemiologischer Stamm identifiziert werden.
Die Ergebnisse dieser Studie sind jedoch nur eingeschränkt auf ein großes Kollektiv projizierbar, da der Untersuchungszeitraum insgesamt nur 7 Jahre betrug und sich das Patientenkollektiv auf das Einzugsgebiet des Klinikums beschränkte.
Untersuchungen zur Apoptose durch das Kontakthapten NiCl 2 in humanen Nabelschnurendothelzellen

Ziel unserer Untersuchungen war es, herauszufinden, ob das Kontakthapten und Umweltgift Nickelchlorid im Vergleich zum etablierten Apoptoseinduktor TNF programmierten Zelltod in Endothelzellen auslösen kann. In Gegenwart des Proteinsyntheseinhibitors Cycloheximid sowie des Transkritionsinhibitors Actinomycin D zeigte sich eine dosisabhängige Zunahme der Apotposerate, die offenbar eine Inhibition proteinsyntheseabhängiger und somit vor Apoptose schützender Mechanismen erfordert. Die Synthese dieser zytoprotektiven Proteine ist von einer Aktivierung des Transkriptionsfaktors NF-kB abhängig, wie es auch nach Exposition mit Nickel beobachtet wird. Zur Untersuchung der Mechanismen der NiCl-vermittelten Apoptose setzten wir weiterhin den Caspaseinhibitor Z-VADfmk ein; dieser blockierte die NiCl/CHX-vermittelte DNA-Fragmentation und Apoptose vollständig. Im Rahmen physiologischer wie auch pathophysiologischer Vorgänge werden Endothelzellen oxidativem Streß in Form reaktiver Sauerstoffradikale ausgesetzt, die zu einer Heraufregulation von Fas und seines Liganden FasL, welche einen bedeutende Rolle bei der Initiierung des programmierten Zelltods spielen, führen. Nickel bewirkt ähnlich wie freie Sauerstoffradikale eine Heraufregulation beider Parameter, erfordert hierfür aber die Gegenwart von CHX. Weiterhin wurde der Einfluß der Mitogen-aktivierbaren (MAP-)-Kinase p38 auf die NiCl-vermittelte DNA-Fragmentation studiert. P38 wird nach Exposition mit Nickel, ähnlich wie nach Stimulation mit TNF, aktiviert; eine Hemmung dieser Kinase mit dem pharmakologischen Inhibitor SB 202190 steigert die Nickelchlorid-induzierte Apoptoserate. Zusammenfassend belegt diese Studie, daß Nickel, welches als Kontaktallergen, als Umweltgift sowie als Bestandteil mancher Prothesematerialien im Kontext der Biokompatibilität medizinisch relevant sein kann, über weitgehend noch undefinierte Signalwege DNA-Fragmentation und Apoptose von primären humanen Endothelzellen vermittelt.
Acne inversa (AI), in der englischen Literatur auch als Hidradenitis suppurativa (HS) bezeichnet, ist eine chronische, entzündliche und schmerzhafte Hautkrankheit, die re- zidivierende Knoten, Fisteln, Abszesse und Vernarbungen vor allem in den Intertrigines verursacht. Es zeigen sich bei Betroffenen neben schwerwiegenden somatischen, auch psychologische Komorbiditäten. Das Ziel der vorliegenden wissenschaftlichen Arbeit war es, an einem an AI erkrankten Patientenkollektiv prospektiv systematisch Komor- biditäten, Lebensqualität und psychische Verfassung zu analysieren und auf Korrelatio- nen zu untersuchen, um daraus Handlungsempfehlungen abzuleiten. Mittels dermatolo- gischer und psychologischer Fragebögen mit Fragebögen zur Lebensqualität wurden pseudonymisierte Daten von 110 Studienteilnehmern gewonnen, statistisch aufbereitet und ausgewertet. Es konnte eine statistisch signifikante Korrelation der Visuellen Ana- logskala Schmerz (VAS-Schmerz) mit dem Hospitality Anxiety Depression Scale (HADS) bzw. dem Skindex-29 aufgezeigt werden. Der Zeitraum zwischen Erstsymp- tomen und Diagnosestellung der AI erfolgte im Median nach 6 Jahren. Weiterhin erfuh- ren AI-Patienten häufig eine nicht leitliniengerechte Therapie und zeigten psychische Belastungen anhand von Schlafstörungen, besonderen Stresssituationen und eine damit einhergehende Verschlechterung der AI. Body Mass Index (BMI), „Waist-to-hip-ratio“ und Bluthochdruck waren oftmals erhöht. Die VAS-Schmerz-Skala könnte ergänzend als Instrument zur ersten Quantifizierung der Krankheitsschwere angewandt werden. Internisten, Chirurgen, Allgemeinmediziner, Gynäkologen und Urologen sollten mit der Dermatose AI besser vertraut gemacht werden, um den Patienten eine schnellere, leitli- niengerechte Therapie zukommen zu lassen. AI-Patienten benötigen neben der dermato- logischen eventuell eine psychologische bzw. psychiatrische Therapie, um das Stressni- veau zu senken, was sich auf die Lebensqualität positiv auswirken könnte.
Hintergrund:Die Aktivierung naiver T-Zellen ist Folge eines Kontaktes zu Antigen präsentierenden Zellen (APC), die auf ihrer Oberfläche Antigene im MHC-Peptid-Komplex präsentieren. Bisherige Daten zur Kontaktdauer und -dynamik sowie zur nachfolgenden Aktivierung und Proliferation naiver T-Zellen beruhen meist auf Ergebnissen von Experimenten, die in Flüssigkulturmodellen gemacht wurden. Aus diesen resultierte die Beschreibung des sog. statischen Interaktionskonzeptes. Die T-Zell-Aktivierung wird überwiegend als Folge eines einzelnen lang dauernden und statischen Kontaktes zwischen T-Zelle und APC beschrieben (single encounter model), der zu einer kontinuierlichen Stimulation des T-Zell-Rezeptors über mehrere Stunden führt. Dem gegenüber steht das Konzept dynamischer Interaktionen, in dem T-Zell-Aktivierung und -Proliferation als Folge dynamischer, kurz dauernder und sequentieller Kontakte zu einer oder zu verschiedenen DC beschrieben werden (serielles Kontaktmodell). Methode: Da Flüssigkeitskulturen jedoch nicht annähernd das dreidimensionale Netzwerk lymphatischer Organe widerspiegeln, in dem der Kontakt zwischen T-Zellen und APC in vivo stattfindet, sollten in der vorliegenden Arbeit naive T-Zellen und dendritische Zellen (DC) in einer dreidimensionalen (3D) Umgebung kokultiviert und auf Interaktionsdynamik und Mitoseaktivität untersucht werden. Im Verlauf der oxidativen Mitogenese mit autologen DC in einer 3D Kollagenmatrix über 56 Stunden wurden humane naive T-Zellen auf Dauer und Dynamik der Zell-Zell-Interaktionen videomikroskopisch sowie die nachfolgende Aktivierung und Proliferation mittels Durchflusszytometrie untersucht. Ergebnisse: Sowohl während der Oxidativen Mitogenese als auch in nicht stimulierten Kontrollkulturen wurden bei naiven T-Zellen fast ausschließlich kurz dauernde, wenige Minuten anhaltende Kontakte zwischen T-Zellen und DC beobachtet. Die mediane Dauer der Kontakte der Kontroll-T-Zellen zu DC war dagegen während aller Beobachtungsintervalle kürzer als die der naiven T-Zellen unter Oxidative Mitogenese-Bedingungen. Es ergaben sich in der Gesamtpopulation der naiven T-Zellen in allen Beobachtungszeiträumen signifikante Unterschiede bezüglich der medianen Interaktionszeiten unter Oxidative Mitogenese-Bedingungen und Kontrollbedingungen Die mediane Interaktionszeit unter Oxidative Mitogenese-Bedingungen lag in allen Beobachtungszeiträumen bei über fünf bis zehn Minuten, unter Kontrollbedingungen jeweils zwischen drei und sechs Minuten (p jeweils < 0,001). Lediglich in der Gruppe der anfangs DC adhärenten T-Zellen nach 24 – 32 Stunden konnten keine signifikanten Unterschiede bezüglich der Dauer der Kontakte festgestellt werden (p = 0,461). Sowohl die Oxidative Mitogenese - wie auch die Kontrollkulturen zeigten nahezu ausschließlich dynamische und serielle Kontakte zu DC, multizelluläre Aggregate und statische Kontakte traten dagegen nur sehr selten auf. Infolge der Oxidativen Mitogenese, nicht jedoch in Kontrollkulturen, traten 40 %der T-Zellen in den Zellzyklus und durchliefen bis zu sechs Mitosen innerhalb von 96 h. Ausblick: Die Oxidative Mitogenese ist ein suffizientes Modell der vollständigen Aktivierung humaner peripherer naiver T-Lymphozyten in Kokultivierung mit DC. Passend zu in vivo Befunden sowie in vitro Daten muriner TCR-transgener T-Zellen erfolgt die Aktivierung und nachfolgende Proliferation überwiegend durch dynamische und kurzlebige Interaktionen. Die vorliegende Arbeit bestätigt das serielle Kontaktmodell für die Aktivierung naiver humaner T-Zellen.
Merkel cell carcinoma (MCC) is a deadly skin cancer, and about 80% of its cases have been shown to harbor integrated Merkel polyomavirus in the tumor cell genome. Viral oncoproteins expressed in the tumor cells are considered as the oncogenic factors of these virus-positive Merkel cell carcinoma (VP-MCC). In contrast, the molecular pathogenesis of virus-negative MCC (VN-MCC) is less well understood. Using gene expression analysis of MCC cell lines, we found histone methyltransferase PRDM8 to be elevated in VN-MCC. This finding was confirmed by immunohistochemical analysis of MCC tumors, revealing that increased PRDM8 expression in VN-MCC is also associated with increased H3K9 methylation. CRISPR-mediated silencing of PRDM8 in MCC cells further supported the histone methylating role of this protein in VN-MCC. We also identified miR-20a-5p as a negative regulator of PRDM8. Taken together, our findings provide insights into the role of PRDM8 as a histone methyltransferase in VN-MCC tumorigenesis.
Tumour progression stage-dependent secretion of YB-1 stimulates melanoma cell migration and invasion

Secreted factors play an important role in intercellular communication. Therefore, they are not only indispensable for the regulation of various physiological processes but can also decisively advance the development and progression of tumours. In the context of inflammatory disease, Y-box binding protein 1 (YB-1) is actively secreted and the extracellular protein promotes cell proliferation and migration. In malignant melanoma, intracellular YB-1 expression increases during melanoma progression and represents an unfavourable prognostic marker. Here, we show active secretion of YB-1 from melanoma cells as opposed to benign cells of the skin. Intriguingly, YB-1 secretion correlates with the stage of melanoma progression and depends on a calcium- and ATP-dependent non-classical secretory pathway leading to the occurrence of YB-1 in the extracellular space as a free protein. Along with an elevated YB-1 secretion of melanoma cells in the metastatic growth phase, extracellular YB-1 exerts a stimulating effect on melanoma cell migration, invasion, and tumourigenicity. Collectively, these data suggest that secreted YB-1 plays a functional role in melanoma cell biology, stimulating metastasis, and may serve as a novel biomarker in malignant melanoma that reflects tumour aggressiveness.
5.1 Immuntherapie mit vom Tumorstroma abgeleiteten Peptiden Tumore bestehen nicht nur aus Tumorzellen, sondern auch aus der sie umgebenden extrazellulären Matrix (EZM), und Stromazellen wie Fibroblasten (cancer-associated fibroblast; CAF) und Endothelzellen (tumor endothelial cell; TEC). Diese Stromazellen haben durch die Ausschüttung von Zytokinen, proteolytischen Enzymen, Wachstums- und Angiogenesefaktoren einen entscheidenden Einfluss auf die Tumorprogression. Sie unterscheiden sich von den Stromazellen der normalen Gewebe durch die Expression von sogenannten Tumorstroma-assoziierten Antigenen (TSAA). Damit sollten Therapien, die auf TSAA abzielen, universell einsetzbar und weniger anfällig gegenüber Resistenzentwicklungen (immune escape Mechanismen) sein, da Stromazellen im Gegensatz zu neoplastischen Zellen genetisch relativ stabil sind. Für eine Immuntherapie mit vom Tumorstroma abgeleiteten Peptiden wählten wir die TSAA Endoglin und Fap, welche während der Wundheilung und im Tumorstroma induziert werden. Dabei sollte überprüft werden, ob prophylaktische Vakzinierungen in C57Bl/6j Mäusen Peptid-reaktive T-Zellen induzieren können, und das Wachstum von transplantieren Grm1-transgenen Tumoren reduziert werden kann. In der Tat konnten wir sowohl bei Endoglin- als auch bei Fap Peptid vakzinierten Tieren in vivo Peptid-reaktive Lymphozyten im Blut und zu einem geringeren Anteil auch in der Milz nachweisen, welche Peptid-gepulste syngene Milzzellen lysieren konnten. Allerdings konnte in beiden Fällen keine Reduktion des Tumorwachstums gegenüber der Kontrollgruppe beobachtet werden. Bei der Fap-Peptid-vakzinierten Gruppe war das Tumorwachstum gegenüber der Kontrollgruppe sogar gesteigert. Dies könnte darauf hindeuten, dass die Induktion Fap-Peptid-reaktiver T-Zellen tumorpromovierend wirkt. Möglicherweise könnte aber durch eine Modifikation des Vakzinierungsprotokolls bzw. durch eine Kombination mit anderen Immuntherapeutika ein verbessertes Ansprechen auf eine Endoglin bzw. Fap basierte Immuntherapie erzielt werden. 5.2 Immunsuppressive Mechanismen im Grm1-transgenen Melanom-Modell Grm1-transgene Mäuse entwickeln spontan kutane Melanome. Dieses Modell erlaubte es uns in der vorliegenden Arbeit spontane Immunantworten im Laufe der Melanomentstehung zu untersuchen. Hierfür analysierten wir sowohl ex vivo als auch in vitro aus Milz und Lymphknoten gewonnene Lymphozyten von Mäusen, welche keine Tumorläsionen bzw. eine niedrige oder hohe Tumorlast aufwiesen. Dabei konnten wir ex vivo einen Anstieg der Frequenz aktivierter CD4+ und CD8+ Lymphozyten mit zunehmender Tumorlast zeigen. Bei tumortragenden Tieren exprimierten jedoch hauptsächlich CD4+ T-Zellen Aktivierungsmarker nach in vitro Stimulation. Interessanterweise waren diese Zellen tumortragender Tiere auch funktionell beeinträchtigt, was sich in einer verminderten Proliferationskapazität nach in vitro Stimulation zeigte. Weitere Analysen ergaben, dass die erhöhte Frequenz regulatorischer T Zellen bei tumortragenden Tieren ein frühes Ereignis im Laufe der Tumorentstehung ist. Gleichzeitig konnte auch ein starker Anstieg der immunsupprimierenden Zytokine Tgf-β1 und Il-10 sowohl in den Lymphknoten als auch im Tumorgewebe beobachtet werden. Dabei war die Tgf-β1-Expression sowohl im Tumor als auch im tumor-drainierenden Lymphknoten erhöht, während Il-10 im Tumor nur moderat exprimiert wurde, was eine komplexere Regulation der Il-10-Expression nahe legt. Dies bedeutet, dass in Grm1-transgenen Mäusen ähnlich wie auch bei Melanompatienten zelluläre und zytokinabhängige Mechanismen zur Tumorentstehung beitragen und dieses Modell daher geeignet ist, um präklinisch immunmodulierende Therapieansätze zu testen.
Ein Tumor stellt nicht nur eine Ansammlung entarteter Zellen dar, sondern ist vielmehr ein komplexes Pseudoorgan, das aus Tumorzellen und aus mit ihnen assoziierten „normalen“ Zelltypen, wie Fibroblasten, Endothelzellen und Makrophagen, den sogenannten Tumorstromazellen, besteht. Die Tumorstromazellen wurden von den Tumorzellen dahingehend konditioniert, dass sie das Tumorwachstum und -progression fördern. Im Rahmen der vorliegenden Arbeit wurde die Bedeutung von zwei Oberflächenmolekülen, nämlich CD147 und CD28, für solche im Tumorstroma stattfindenden Interaktionen im syngenen murinen B16 Melanommodell untersucht. Rolle von CD147 für MMP Expression, Neoangiogenese und Metastasierung CD147, das von Tumorzellen exprimiert wird, wird als ein Faktor angesehen, der auf benachbarten Stromazellen die Expression von MMPs induziert. MMPs sind essentiell für den Umbau der extrazellulären Matrix und der Basalmembranen und somit für die Invasion und Metastasierung des Tumors essentiell. Daneben gibt es erste Hinweise, dass CD147 auch die Induktion von vascular endothelial growth factor (VEGF) vermittelt und damit die Tumorangiogenese fördert. In dem eingesetzten Melanommodell war überraschenderweise kein Unterschied hinsichtlich der Expression von MMP-2, MMP-9 und MT1-MMP in Abhängigkeit von der CD147 Expression nachweisbar. Die in vitro Kokultur der Melanomzellen mit unterschiedlichen murinen Fibroblasten zeigte zudem, dass weder CD147+ noch CD147- Melanomzellen die Expression von MMP-2 oder MMP-9 in den Fibroblasten veränderten. Als eindeutige Effekte des CD147 knock downs wurde aber eine reduzierte VEGF Expression in vivo einhergend mit einer gehemmten Tumorangiogenese, sowie einer reduzierten Metastasierung festgestellt. Es konnte somit die Funktion von CD147 in dem gewählten Modell als angiogenetischer, jedoch als MMP unabhängiger, Metastasierungsfaktor demonstriert werden. Einfluss von CD28 auf antitumorale Immunantworten CD28 ist ein kostimulatorisches Molekül, das zusammen mit dem TCR für eine effiziente Stimulation von T-Lymphozyten wesentlich ist. In CD28 k.o Mäusen fand sich im Vergleich zu Wildtyp Kontrolltieren eine verminderte Effektivität von prophylaktischen anti-Tumor Impfungen, die sich in einem beschleunigten Tumorwachstum sowie einer erhöhten Tumorlast auswirkten. Die Frequenz von Vakzine induzierten TRP-2180-188 /Kb reaktiven CD8+ T-Zellen in TIL von Tumoren war aber in beiden Genotypen gleich. Dagegen war die Anzahl IFN- produzierender TRP-2180-188 /Kb reaktiver T-Zellen sowie die Fähigkeit der TRP-2180-188 /Kb reaktiven T-Zellen zu lysieren, in den CD28-defizienten Mäusen deutlich geringer. Diese Beobachtungen legen nahe, dass CD28-vermittelte kostimulatorische Signale im gewählten Modell weniger für die initiale Expansion als für die Differenzierung funktioneller tumorspezifischer CD8+ T-Effektorzellen eine wesentliche Funktion einzunehmen scheinen.
Immune checkpoint blockade therapy is beneficial and even curative for some cancer patients. However, the majority don’t respond to immune therapy. Across different tumor types, pre-existing T cell infiltrates predict response to checkpoint-based immunotherapy. Based on in vitro pharmacological studies, mouse models and analyses of human melanoma patients, we show that the cytokine GDF-15 impairs LFA-1/β2-integrin-mediated adhesion of T cells to activated endothelial cells, which is a pre-requisite of T cell extravasation. In melanoma patients, GDF-15 serum levels strongly correlate with failure of PD-1-based immune checkpoint blockade therapy. Neutralization of GDF-15 improves both T cell trafficking and therapy efficiency in murine tumor models. Thus GDF-15, beside its known role in cancer-related anorexia and cachexia, emerges as a regulator of T cell extravasation into the tumor microenvironment, which provides an even stronger rationale for therapeutic anti-GDF-15 antibody development.
Background
Pemphigus vulgaris and pemphigus foliaceus are potentially life-threatening autoimmune disorders triggered by IgG autoantibodies against mucosal and epidermal desmogleins. There is an unmet need for fast-acting drugs that enable patients to achieve early sustained remission with reduced corticosteroid reliance.
Objectives
To investigate efgartigimod, an engineered Fc fragment that inhibits the activity of the neonatal Fc receptor, thereby reducing serum IgG levels, for treating pemphigus.
Methods
Thirty-four patients with mild-to-moderate pemphigus vulgaris or foliaceus were enrolled in an open-label phase II adaptive trial. In sequential cohorts, efgartigimod was dosed at 10 or 25 mg kg\(^{-1}\) intravenously with various dosing frequencies, as monotherapy or as add-on therapy to low-dose oral prednisone. Safety endpoints comprised the primary outcome. The study is registered at ClinicalTrials.gov (identifier NCT03334058).
Results
Adverse events were mostly mild and were reported by 16 of 19 (84%) patients receiving efgartigimod 10 mg kg\(^{-1}\) and 13 of 15 (87%) patients receiving 25 mg kg−1, with similar adverse event profiles between dose groups. A major decrease in serum total IgG and anti-desmoglein autoantibodies was observed and correlated with improved Pemphigus Disease Area Index scores. Efgartigimod, as monotherapy or combined with prednisone, demonstrated early disease control in 28 of 31 (90%) patients after a median of 17 days. Optimized, prolonged treatment with efgartigimod in combination with a median dose of prednisone 0·26 mg \(^{-1}\) per day (range 0·06–0·48) led to complete clinical remission in 14 of 22 (64%) patients within 2–41 weeks.
Conclusions
Efgartigimod was well tolerated and exhibited an early effect on disease activity and outcome parameters, providing support for further evaluation as a therapy for pemphigus.
The relevance of the adaptor protein TNF receptor-associated factor 2 (TRAF2) for signal transduction of the death receptor tumour necrosis factor receptor1 (TNFR1) is well-established. The role of TRAF2 for signalling by CD95 and the TNF-related apoptosis inducing ligand (TRAIL) DRs, however, is only poorly understood. Here, we observed that knockdown (KD) of TRAF2 sensitised keratinocytes for TRAIL- and CD95L-induced apoptosis. Interestingly, while cell death was fully blocked by the pan-caspase inhibitor benzyloxycarbonyl-Val-Ala-Asp(OMe)-fluoromethylketone (zVAD-fmk) in control cells, TRAF2-depleted keratinocytes were only partly rescued from TRAIL- and CD95L-induced cell death. In line with the idea that the only partially protective effect of zVAD-fmk on TRAIL- and CD95L-treated TRAF2-depleted keratinocytes is due to the induction of necroptosis, combined treatment with zVAD-fmk and the receptor interacting protein 1 (RIP1) inhibitor necrostatin-1 fully rescued these cells. To better understand the impact of TRAF2 levels on RIP1- and RIP3-dependent necroptosis and RIP3-independent apoptosis, we performed experiments in HeLa cells that lack endogenous RIP3 and HeLa cells stably transfected with RIP3. HeLa cells, in which necroptosis has no role, were markedly sensitised to TRAIL-induced caspase-dependent apoptosis by TRAF2 KD. In RIP3-expressing HeLa transfectants, however, KD of TRAF2 also strongly sensitised for TRAIL-induced necroptosis. Noteworthy, priming of keratinocytes with soluble TWEAK, which depletes the cytosolic pool of TRAF2-containing protein complexes, resulted in strong sensitisation for TRAIL-induced necroptosis but had only a very limited effect on TRAIL-induced apoptosis. The necroptotic TRAIL response was not dependent on endogenously produced TNF and TNFR signalling, since blocking TNF by TNFR2-Fc or anti-TNFα had no effect on necroptosis induction. Taken together, we identified TRAF2 not only as a negative regulator of DR-induced apoptosis but in particular also as an antagonist of TRAIL- and CD95L-induced necroptosis.
Die vorliegende Promotionsarbeit beschäftigte sich mit der Frage, ob der Todesligand TRAIL in Keratinozyten eine Aktivierung verschiedener Mitogen-aktivierter Protein Kinasen (MAPK) induzieren kann und welche physiologische Relevanz diese TRAIL-induzierte MAPK-Aktivität hat. In unseren Analysen konnte nachgewiesen werden, dass TRAIL die MAPKERK1/2, MAPKJNK1/2 und MAPKp38 mit unterschiedlicher Kinetik aktivieren kann. Diese Aktivierung zeigte sich beeinflusst vom verwendeten Zelltyp, der Zeitdauer der Stimulation sowie dem Ausmaß der TRAIL-induzierten Caspase-Aktivität. Die TRAIL-vermittelte Aktivierung der MAPKERK1/2 beginnt sehr rasch und kann über einen längeren Zeitraum detektiert werden, während die MAPKJNK erst spät aktiviert wird. Im Gegensatz dazu zeigt die MAPKp38 eine biphasische Aktivierung. Die TRAIL-induzierte Aktivierung der MAPK ist teilweise von aktiven Caspasen abhängig, denn eine Präinkubation mit dem pharmakologischen Caspase-Inhibitor zVAD-fmk hemmt sowohl die TRAIL-induzierte MAPKJNK- als auch die MAPKp38-Aktivität. Untersuchungen mit ektoper Expression des physiologischen Caspase-8 Inhibitors c-FLIPL konnten zeigen, dass cFLIPL nicht nur die Spaltung von Caspase-8, sondern auch die verzögerte TRAIL-induzierte MAPKp38-Aktivität hemmen kann. In der vorliegenden Arbeit wurde außerdem nachgewiesen, dass TRAIL in Keratinozyten nicht nur Apoptose induziert, sondern auch an der Sekretion des proinflammatorischen Chemokins CXCL-8 beteiligt ist. Dabei war die MAPKp38, aber nicht die MAPKERK1/2 an der TRAIL-induzierten Sekretion von CXCL-8 beteiligt. Zukünftig werden weitere detailliertere Untersuchungen insbesondere zur physiologischen Bedeutung der TRAIL-induzierten MAPKJNK- und MAPKERK1/2-Aktivität erforderlich sein, für die diese Arbeit eine wichtige Grundlage gelegt hat.
Für Patienten mit malignem Melanom im Stadium der Fernmetastasierung gibt es bis heute lediglich Therapieoptionen mit sehr eingeschränkten Erfolgsaussichten. Diese Tatsache bestätigt die Notwendigkeit von Biomarkern zur Vorhersage des Erfolgs verschiedener Therapien. Der ATP-basierende ex vivo Chemosensitivitätsassay hat sich als erfolgreiche Methode zur individuellen Vorhersage eines Chemotherapieerfolgs herausgestellt. Tatsächlich zeigte der Assay ein heterogenes Sensitivitätsprofil gegen verschiedene Chemotherapeutika und ließ in getesteten Patienten ein ex vivo wirksames Chemotherapieregime identifizieren, das anschließend auch klinische Therapieerfolge bei Verwendung der Therapie mit dem besten individuellen Chemosensitivitätsindex(BICSI) zeigte. Um diesen sehr aufwendigen Assay zukünftig zu umgehen, sollten in der vorliegenden Arbeit prädiktive molekulare Biomarker der Chemosensitivität identifiziert werden. Hierfür wurden im Voraus durch einen Microarray die Kandidaten Secernin 1 (SCRN1), Lysyl oxidaselike 1 (LOXL1), Thymosin beta 4 X-linked (TMSB4X), Vesicle-associated membrane protein 5 (VAMP5) und Serine protease inhibitor B1 (SERPINB1) als differentiell exprimierte Gene in chemosensitivem gegenüber chemoresistentem Gewebe identifiziert. Die relative Expression dieser Kandidatengene wurde daraufhin in bis zu 128 verschiedenen Melanomgeweben mit dem Chemosensitivitätsindex verschiedener Chemotherapeutika korreliert. Hierbei konnte eine signifikante Korrelation zwischen SerpinB1 mit der Chemosensitivität gegenüber der Therapiekombination mit Paclitaxel und Cisplatin auf Gen- aber nicht auf Proteinebene identifiziert werden. Weiterhin konnte eine differentielle Expression ebenfalls in chemosensitiven und -resistenten Melanomzelllinien nachgewiesen werden, die allerdings im Vergleich mit dem analysierten Gewebe in gegensätzlicher Richtung verlief. Zusammenfassend lässt sich sagen, dass SerpinB1 ein vielversprechender Marker für die Chemosensitivität gegenüber Paclitaxel und Cisplatin ist, dessen funktionelle Bedeutung aber unklar bleibt. Das Merkelzellkarzinom (MCC) ist ein seltener und hoch aggressiver Tumor der mit dem Merkelzellpolyomavirus (MCV) in Zusammenhang steht. Da MCC Zelllinien zur Aufrechterhaltung ihrer Viabilität die MCV T-Antigene benötigen, könnte der Einsatz von Interferonen (IFN) ein möglicher therapeutischer Ansatz zur Behandlung dieser Krebserkrankung sein. In der vorliegenden Arbeit haben wir daher die Effekte von IFNs auf MCC Zelllinien, mit besonderer Berücksichtigung der MCV+ Linien, untersucht. IFNs vom Typ I (hier Multiferon, ein Mix verschiedener IFN α Subtypen, und IFN β) wirkten stark inhibierend auf die zelluläre Viabilität. Die Zellzyklusanalyse zeigte eine Erhöhung des sub-G Anteils der Zellen nach Behandlung mit IFN, was auf Apoptose als ausschlagebenden Grund schließen ließ. Diese Effekte waren für die Behandlung mit IFN β weniger stark ausgeprägt. Der inhibitorische Effekt von Typ I IFNs auf MCV+ MCC Zelllinien war assoziiert mit einer verringerten Expression des viralen großen T-Antigens (LTA) und einer Erhöhung in der Expression von promyelocytic leukemia protein (PML), das dafür bekannt ist, die Funktion des LTA störend zu beeinflussen. Zusätzlich führte die intratumorale Anwendung von Multiferon in vivo zu einer Regression im Wachstum von MCV+, aber nicht MCV- MCC Xenotransplantaten. Die Ergebnisse zeigen das Typ I IFNs einen starken antitumoralen Effekt haben, der zum Teil durch die Regulierung des LTA herbeigeführt wird. Neben diesen direkten Effekten der IFNs auf die Zellproliferation induzieren diese auch die Expression von MHC Klasse I Molekülen in MCC Zelllinien. Die Durchflusszytometrie zeigte eine Induktion der MHC Klasse I Expression in drei MHC I negativen MCC Zelllinien und eine Erhöhung der Expression, die vor der Behandlung eine geringe Menge an MHC I aufwiesen. Diese Effekte konnten auch in den in vivo Xenotransplantaten beobachtet werden. Die Ergebnisse zeigen, dass die Behandlung mit IFN sowohl direkte als auch indirekte Effekte auf das MCC hat und eine breite Anwendung in Patienten mit MCV+ und MCV- Tumoren finden kann. Neben IFNs sind auch Artemisinin und seine Derivate bekannt für ihre antitumoralen und antiviralen Eigenschaften. Daher haben wir den Effekt des Artemisininderivats Artesunate auf MCV+ und MCV- MCC Zelllinien getestet. Tatsächlich konnten wir auch hier einen antiproliferativen Effekt des Stoffes nachweisen, der stärker auf MCV+ als auf MCV- Zelllinien wirkte und bei ersteren wiederum mit einer reduzierten LTA Expression einherging. Im Vergleich dazu blieben Fibroblasten von der Behandlung unbeeinflusst. Das verringerte Tumorwachstum konnte ebenfalls für in vivo Xenotransplantationsmodelle gezeigt werden. Auf Grundlage dieser Erkenntnis sollte eine genauere Untersuchung dieses alten Naturheilstoffes für die Behandlung von MCC Patienten in Betracht gezogen werden.
The incidence of melanoma and nonmelanoma skin cancer has increased tremendously in recent years. Although novel treatment options have significantly improved patient outcomes, the prognosis for most patients with an advanced disease remains dismal. It is, thus, imperative to understand the molecular mechanisms involved in skin carcinogenesis in order to develop new targeted treatment strategies. Receptor tyrosine kinases (RTK) like the ERBB receptor family, including EGFR/ERBB1, ERBB2/NEU, ERBB3, and ERBB4, are important regulators of skin homeostasis and their dysregulation often results in cancer, which makes them attractive therapeutic targets. Members of the leucine‐rich repeats and immunoglobulin‐like domains protein family (LRIG1‐3) are ERBB regulators and thus potential therapeutic targets to manipulate ERBB receptors. Here, we analyzed the function of LRIG1 during chemically induced skin carcinogenesis in transgenic mice expressing LRIG1 in the skin under the control of the keratin 5 promoter (LRIG1‐TG mice). We observed a significant induction of melanocytic tumor formation in LRIG1‐TG mice and no difference in papilloma incidence between LRIG1‐TG and control mice. Our findings also revealed that LRIG1 affects ERBB signaling via decreased phosphorylation of EGFR and increased activation of the oncoprotein ERBB2 during skin carcinogenesis. The epidermal proliferation rate was significantly decreased during epidermal tumorigenesis under LRIG1 overexpression, and the apoptosis marker cleaved caspase 3 was significantly activated in the epidermis of transgenic LRIG1 mice. Additionally, we detected LRIG1 expression in human cutaneous squamous cell carcinoma and melanoma samples. Therefore, we depleted LRIG1 in human melanoma cells (A375) by CRISPR/Cas9 technology and found that this caused EGFR and ERBB3 downregulation in A375 LRIG1 knockout cells 6 h following stimulation with EGF. In conclusion, our study demonstrated that LRIG1‐TG mice develop melanocytic skin tumors during chemical skin carcinogenesis and a deletion of LRIG1 in human melanoma cells reduces EGFR and ERBB3 expression after EGF stimulation.
The transcription factor NRF2 is the major mediator of oxidative stress responses and is closely connected to therapy resistance in tumors harboring activating mutations in the NRF2 pathway. In melanoma, such mutations are rare, and it is unclear to what extent melanomas rely on NRF2. Here we show that NRF2 suppresses the activity of the melanocyte lineage marker MITF in melanoma, thereby reducing the expression of pigmentation markers. Intriguingly, we furthermore identified NRF2 as key regulator of immune-modulating genes, linking oxidative stress with the induction of cyclooxygenase 2 (COX2) in an ATF4-dependent manner. COX2 is critical for the secretion of prostaglandin E2 and was strongly induced by H\(_2\)O\(_2\) or TNFα only in presence of NRF2. Induction of MITF and depletion of COX2 and PGE2 were also observed in NRF2-deleted melanoma cells in vivo. Furthermore, genes corresponding to the innate immune response such as RSAD2 and IFIH1 were strongly elevated in absence of NRF2 and coincided with immune evasion parameters in human melanoma datasets. Even in vitro, NRF2 activation or prostaglandin E2 supplementation blunted the induction of the innate immune response in melanoma cells. Transcriptome analyses from lung adenocarcinomas indicate that the observed link between NRF2 and the innate immune response is not restricted to melanoma.
IgG3 is the IgG subclass with the strongest effector functions among all four IgG subclasses and the highest degree of allelic variability among all constant immunoglobulin genes. Due to its genetic position, IgG3 is often the first isotype an antibody switches to before IgG1 or IgG4. Compared with the other IgG subclasses, it has a reduced half-life which is probably connected to a decreased affinity to the neonatal Fc receptor (FcRn). However, a few allelic variants harbor an amino acid replacement of His435 to Arg that reverts the half-life of the resulting IgG3 to the same level as the other IgG subclasses. Because of its functional impact, we hypothesized that the p.Arg435His variation could be associated with susceptibility to autoantibody-mediated diseases like pemphigus vulgaris (PV) and bullous pemphigoid (BP). Using a set of samples from German, Turkish, Egyptian, and Iranian patients and controls, we were able to demonstrate a genetic association of the p.Arg435His variation with PV risk, but not with BP risk. Our results suggest a hitherto unknown role for the function of IgG3 in the pathogenesis of PV.
Pigment cells and neuronal cells both are derived from the neural crest. Here, we describe the Pit-Oct-Unc (POU) domain transcription factor Brn3a, normally involved in neuronal development, to be frequently expressed in melanoma, but not in melanocytes and nevi. RNAi-mediated silencing of Brn3a strongly reduced the viability of melanoma cell lines and decreased tumour growth in vivo. In melanoma cell lines, inhibition of Brn3a caused DNA double-strand breaks as evidenced by Mre11/Rad50-containing nuclear foci. Activated DNA damage signalling caused stabilization of the tumour suppressor p53, which resulted in cell cycle arrest and apoptosis. When Brn3a was ectopically expressed in primary melanocytes and fibroblasts, anchorage-independent growth was increased. In tumourigenic melanocytes and fibroblasts, Brn3a accelerated tumour growth in vivo. Furthermore, Brn3a cooperated with proliferation pathways such as oncogenic BRAF, by reducing oncogene-induced senescence in non-malignant melanocytes. Together, these results identify Brn3a as a new factor in melanoma that is essential for melanoma cell survival and that promotes melanocytic transformation and tumourigenesis.
The MEK5/ERK5 mitogen-activated protein kinases (MAPK) cascade is a unique signaling module activated by both mitogens and stress stimuli, including cytokines, fluid shear stress, high osmolarity, and oxidative stress. Physiologically, it is mainly known as a mechanoreceptive pathway in the endothelium, where it transduces the various vasoprotective effects of laminar blood flow. However, it also maintains integrity in other tissues exposed to mechanical stress, including bone, cartilage, and muscle, where it exerts a key function as a survival and differentiation pathway. Beyond its diverse physiological roles, the MEK5/ERK5 pathway has also been implicated in various diseases, including cancer, where it has recently emerged as a major escape route, sustaining tumor cell survival and proliferation under drug stress. In addition, MEK5/ERK5 dysfunction may foster cardiovascular diseases such as atherosclerosis. Here, we highlight the importance of the MEK5/ERK5 pathway in health and disease, focusing on its role as a protective cascade in mechanical stress-exposed healthy tissues and its function as a therapy resistance pathway in cancers. We discuss the perspective of targeting this cascade for cancer treatment and weigh its chances and potential risks when considering its emerging role as a protective stress response pathway.
Multiple myeloma (MM) displays an NFκB activity-related gene expression signature and about 20% of primary MM samples harbor genetic alterations conducive to intrinsic NFκB signaling activation. The relevance of blocking the classical versus the alternative NFκB signaling pathway and the molecular execution mechanisms involved, however, are still poorly understood. Here, we comparatively tested NFκB activity abrogation through TPCA-1 (an IKK2 inhibitor), BAY 11-7082 (an IKK inhibitor poorly selective for IKK1 and IKK2), and MLN4924 (an NEDD8 activating enzyme (NAE)-inhibitor), and analyzed their anti-MM activity. Whereas TPCA-1 interfered selectively with activation of the classical NFκB pathway, the other two compounds inhibited classical and alternative NFκB signaling without significant discrimination. Noteworthy, whereas TPCA-1 and MLN4924 elicited rather mild anti-MM effects with slight to moderate cell death induction after 1 day BAY 11-7082 was uniformly highly toxic to MM cell lines and primary MM cells. Treatment with BAY 11-7082 induced rapid cell swelling and its initial effects were blocked by necrostatin-1 or the ROS scavenger BHA, but a lasting protective effect was not achieved even with additional blockade of caspases. Because MLN4924 inhibits the alternative NFκB pathway downstream of IKK1 at the level of p100 processing, the quite discordant effects between MLN4924 and BAY 11-7082 must thus be due to blockade of IKK1-mediated NFκB-independent necrosis-inhibitory functions or represent an off-target effect of BAY 11-7082. In accordance with the latter, we further observed that concomitant knockdown of IKK1 and IKK2 did not have any major short-term adverse effect on the viability of MM cells.
Merkel Cell Carcinoma (MCC) is a rare and highly aggressive neuroendocrine skin cancer for which no effective treatment is available. MCC represents a human cancer with the best experimental evidence for a causal role of a polyoma virus. Large T antigens (LTA) encoded by polyoma viruses are oncoproteins, which are thought to require support of cellular heat shock protein 70 (HSP70) to exert their transforming activity. Here we evaluated the capability of MAL3-101, a synthetic HSP70 inhibitor, to limit proliferation and survival of various MCC cell lines. Remarkably, MAL3-101 treatment resulted in considerable apoptosis in 5 out of 7 MCC cell lines. While this effect was not associated with the viral status of the MCC cells, quantitative mRNA expression analysis of the known HSP70 isoforms revealed a significant correlation between MAL3-101 sensitivity and HSC70 expression, the most prominent isoform in all cell lines. Moreover, MAL3-101 also exhibited in vivo antitumor activity in an MCC xenograft model suggesting that this substance or related compounds are potential therapeutics for the treatment of MCC in the future.
We herein present a patient with delayed-type allergic hypersensitivity against prilocaine leading to spreading eczematous dermatitis after subcutaneous injections for local anesthesia with prilocaine. Prilocaine allergy was proven by positive skin testing and subcutaneous provocation, whereas the evaluation of other local anesthetics - among them lidocaine, articaine and mepivacaine - did not exhibit any evidence for cross-reactivity. Interestingly, our patient repeatedly tolerated strictly deep subcutaneous injection of prilocaine in provocation testing while patch and superficial subcutaneous application mounted strong allergic responses. We hypothesize, that lower DC density in deeper cutaneous compartments and/or different DC subsets exhibiting distinct functional immunomodulatory properties in the various layers of the skin may confer to the observed absence of clinical reactivity against prilocaine after deep subcutaneous injection. The term compartment allergy indicates that the route of allergen administration together with the targeted immunologic environment orchestrates on the immunologic outcome: overt T-cell mediated allergy or clinical tolerance.
Introduction: Large-cell transformation (LCT) of mycosis fungoides (MF) has been associated with a higher risk of relapse and progression and, consequently, restricted prognosis. Its molecular pathogenesis has not been elucidated yet. Materials and Methods: In order to address molecular mechanisms of LCT, we performed hybrid capture panel-based sequencing of skin biopsies from 10 patients suffering from MF with LCT versus 17 patients without LCT including follow-up biopsies during clinical course, respectively (51 samples in total). The analyzed patients were attributed to three different groups based on the presence of LCT and clinical behavior. Results: While indolent MF cases without LCT did not show pathogenic driver mutations, a high rate of oncogenic alterations was detected in patients with LCT and aggressive clinical courses. Various genes of different oncogenic signaling pathways, including the MAPK and JAK-STAT signaling pathways, as well as epigenetic modifiers were affected. A high inter-individual and distinctive intra-individual mutation diversity was observed. Oncogenic RAS mutations were exclusively detected in patients with LCT. Conclusion: Our data demonstrate that LCT transition of MF is associated with increased frequency of somatic mutations in cancer-associated genes. In particular, the activation of RAS signaling — together with epigenetic dysregulation — may crucially contribute to the molecular pathogenesis of the LCT phenotype, thus conveying its adverse clinical behavior.
T-Zellimmunantworten werden normalerweise durch folgenden Weg initiiert: unreife dendritische Zellen nehmen Antigen in der Peripherie auf, wandern in die sekundären lymphatischen Organe, wobei sie auf ihrem Weg sowohl reifen als auch das Antigen prozessieren. In den sekundären lymphatischen Organen angekommen, präsentieren sie als reife dendritische Zellen den T-Zellen die Antigene in Form von Peptiden zusammen mit kostimulierenden Molekülen. Dadurch rufen sie eine spezifische T-Zellantwort hervor. In der vorliegenden Arbeit wurde untersucht, ob nicht Situationen herbeigeführt werden können, die ein T-Zell priming außerhalb der sekundären lymphatischen Organe erlauben. Dazu wurden ein murines Modell, bei dem das Zytokin Lymphotoxin-alpha spezifisch am Tumor angereichert wurde, und ein humanes Modell, bei dem reife, antigenbeladene DC intradermal appliziert wurden, untersucht. Im murinen Modell zeigte sich, dass die gerichtete Anreicherung von Lymphotoxin-alpha am Tumor zu dessen Zerstörung führte, welche durch T-Zellen vermittelt wurde, und mit der Induktion eines tertiären lymphatischen Gewebes am Tumor assoziiert war. Dieses tertiäre lymphatische Gewebe war durch die Kompartimentalisierung von T- und B-Zellen und der Präsenz von high endothelial venules charakterisiert und besaß zudem mit dendritischen Zellen und naïven T-Zellen alle Voraussetzungen für ein in loco priming. Dementsprechend konnte in der Folge der gerichteten Lymphotoxin-alpa Therapie im Tumor ein Anstieg am T-Zellinfiltrat, welches sich oligoklonal zusammensetzte, beobachtet werden. In vitro Experimente verdeutlichte die Tumorspezifität der Therapie-induzierten T-Zellantwort, da die T-Zellen auf ein Tumorantigen mit der Ausschüttung von Interferon gamma reagierten und die Tumorzellen lysierten. Im humanen Modell wurden Hautbiopsien von Melanompatienten untersucht, denen im Rahmen einer klinischen Studie autologe, in vitro generierte und antigenbeladene DC intradermal appliziert wurden. Die Patienten erlaubten die Entnahme von Hautbiopsien aus den Injektionsstellen für wissenschaftliche Untersuchungen. Eine Induktion bzw. Verstärkung einer spezifischen T-Zellantwort durch die Vakzinierung mit antigenbeladenen dendritischen Zellen konnte bereits in zahlreichen Arbeiten und auch in dem in dieser Arbeit untersuchten Patientenkollektiv gezeigt werden. Bei der Analyse der Injektionsstellen zeigt sich, dass ein großer Teil der injizierten dendritischen Zellen in der Vakzinierungsstelle verharren und dass diese unabhängig von einer Beladung mit Antigen zu einer Induktion von high endothelial venules Charakteristika führte. Waren die dendritischen Zellen mit Antigen beladen, so führte dies zu einem stärkeren T-Zellinfiltrat in den Injektionsstellen, wobei sowohl naïve als auch central memory T-Zellen nachgewiesen wurde. Diese Zellen wurden vermutlich durch die Überexpression der DC CK1 und SDF1 Chemokinen in den Injektionsstellen, die chemotaktisch auf T-Zellen wirken, angezogen. Das Infiltrat in den Injektionsstellen war oligoklonal und wies tumorspezifische T-Zellen auf. Nachdem diese T-Zellklone im Blut der Patienten vor der Vakzinierung nicht nachweisbar waren, müssen sie zumindest in den Injektionsstellen expandiert sein. Interessanterweise konnte einer dieser Klone in Metastasen nachgewiesen werden, die nach der Vakzinierung dem Patienten entfernt wurden. In beiden Modellen wurde also durch die Manipulation des Mikromilieus, d.h. Lymphotoxin-alpa Anreicherung am Tumor bzw. Injektion von reifen dendritischen Zellen in die Haut, Strukturen wie z.B. high endothelial venules induziert, die ein in loco priming ermöglichen sollten. Dementsprechend riefen diese Veränderungen ein Tumorantigen-spezifisches Infiltrat hervor. Diese Ergebnisse deuten darauf hin, dass T-Zell priming auch außerhalb sekundärer lymphatischer Organe erfolgen kann. Prinzipiell scheint also nur der Kontakt von reifen, antigenbeladenen dendritischen Zellen mit den entsprechenden antigenspezifischen, naïven T-Zellen entscheiden zu sein. Die Möglichkeit des in vitro primings bekräftigt diese These. In vivo erfolgt dieses Aufeinandertreffen normalerweise in den sekundären lymphatischen Organen, doch konnte in der vorliegenden Arbeit gezeigt werden, dass Veränderungen des Mikromilieus diesen Kontakt auch in anderen Geweben ermöglicht.
The massive infiltration of lymphocytes into the skin is a hallmark of numerous human skin disorders. By co-culturing murine keratinocytes with splenic T cells we demonstrate here that T cells affect and control the synthesis and secretion of chemokines by keratinocytes. While pre-activated CD8\(^+\)T cells induce the synthesis of CXCL9 and CXCL10 in keratinocytes and keep in check the synthesis of CXCL1, CXCL5, and CCL20, keratinocytes dampen the synthesis of CCL3 and CCL4 in pre-activated CD8\(^+\)T cells. One key molecule is IFN-γ that is synthesized by CD8\(^+\)T cells under the control of NFATc1 and NFATc2. CD8\(^+\)T cells deficient for both NFAT factors are unable to induce CXCL9 and CXCL10 expression. In addition, CD8\(^+\)T cells induced numerous type I IFN-inducible “defense genes” in keratinocytes encoding the PD1 and CD40 ligands, TNF-α and caspase-1. The enhanced expression of type I IFN-inducible genes resembles the gene expression pattern at the dermal/epidermal interface in lichen planus, an inflammatory T lymphocyte-driven skin disease, in which we detected the expression of CXCL10 in keratinocytes in close vicinity to the infiltration front of T cells. These data reflect the multifaceted interplay of lymphocytes with keratinocytes at the molecular level.
Systemic treatment of metastatic uveal melanoma: review of literature and future perspectives

Up to 50% of patients with uveal melanoma develop metastatic disease with poor prognosis. Regional, mainly liver-directed, therapies may induce limited tumor responses but do not improve overall survival. Response rates of metastatic uveal melanoma (MUM) to systemic chemotherapy are poor. Insights into the molecular biology of MUM recently led to investigation of new drugs. In this study, to compare response rates of systemic treatment for MUM we searched Pubmed/Web of Knowledge databases and ASCO website (1980–2013) for “metastatic/uveal/melanoma” and “melanoma/eye.” Forty studies (one case series, three phase I, five pilot, 22 nonrandomized, and two randomized phase II, one randomized phase III study, data of three expanded access programs, three retrospective studies) with 841 evaluable patients were included in the numeric outcome analysis. Complete or partial remissions were observed in 39/841 patients (overall response rate [ORR] 4.6%; 95% confidence intervals [CI] 3.3–6.3%), no responses were observed in 22/40 studies. Progression-free survival ranged from 1.8 to 7.2, median overall survival from 5.2 to 19.0 months as reported in 21/40 and 26/40 studies, respectively. Best responses were seen for chemoimmunotherapy (ORR 10.3%; 95% CI 4.8–18.7%) though mainly in first-line patients. Immunotherapy with ipilimumab, antiangiogenetic approaches, and kinase inhibitors have not yet proven to be superior to chemotherapy. MEK inhibitors are currently investigated in a phase II trial with promising preliminary data. Despite new insights into genetic and molecular background of MUM, satisfying systemic treatment approaches are currently lacking. Study results of innovative treatment strategies are urgently awaited.
Since 2021, adalimumab biosimilar ABP 501 can be used alternatively to adalimumab originator (ADAO) in the treatment of hidradenitis suppurativa (HS). Effectiveness and safety data remain scarce. We investigated the impact of switching from ADAO to ABP 501 on disease severity and the occurrence of adverse events (AEs) in patients with HS. We analyzed clinical data on patients enrolled in the German HSBest registry. Evaluation outcomes were assessed at three time points (baseline of originator (t0), prior to switching to biosimilar (t1) and 12 to 14 weeks after switching (t2)) and included patient-reported AEs and disease severity using the International Hidradenitis Suppurativa Severity Score System (IHS4) score. In total, 94 patients were switched from ADAO to ABP 501. Overall, 33.3% (n = 31/94) of the patients developed AEs and/or loss of response (LoR) within 12 to 14 weeks after switching. Of these, 61.3% (n = 19/31) experienced LoR but no AEs, 22.6% (n = 7/31) LoR combined with AEs and 16.1% (n = 5/31) AEs only. Our study showed that switching HS patients from ADAO to ABP 501 does significantly affect treatment effectiveness. Switching patients who are on remission maintenance therapy should be viewed critically.

Therapeutic vaccination directed to induce an anti-tumoral T-cell response is a field of extensive investigation in the treatment of melanoma. However, many vaccination trials in melanoma failed to demonstrate a correlation between the vaccine-specific immune response and therapy outcome. This has been mainly attributed to immune escape by antigen loss, rendering us in the need of new vaccination targets.
Patients and methods
This phase-II trial investigated a peptide vaccination against survivin, an oncogenic inhibitor-of-apoptosis protein crucial for the survival of tumor cells, in HLA-A1/-A2/-B35-positive patients with treatment-refractory stage-IV metastatic melanoma. The study endpoints were survivin-specific T-cell reactivity (SSTR), safety, response, and survival (OS).
Results
Sixty-one patients (ITT) received vaccination therapy using three different regimens. 55 patients (PP) were evaluable for response and survival, and 41/55 for SSTR. Patients achieving progression arrest (CR + PR + SD) more often showed SSTRs than patients with disease progression (p = 0.0008). Patients presenting SSTRs revealed a prolonged OS (median 19.6 vs. 8.6 months; p = 0.0077); multivariate analysis demonstrated SSTR as an independent predictor of survival (p = 0.013). The induction of SSTRs was associated with gender (female vs. male; p = 0.014) and disease stage (M1a/b vs. M1c; p = 0.010), but not with patient age, HLA type, performance status, or vaccination regimen.
Conclusion
Survivin-specific T-cell reactivities strongly correlate with tumor response and patient survival, indicating that vaccination with survivin-derived peptides is a promising treatment strategy in melanoma.

Background: Renal cell carcinoma (RCC) is divided into three major histopathologic groups—clear cell (ccRCC), papillary (pRCC) and chromophobe RCC (chRCC). We performed a comprehensive re-analysis of publicly available RCC datasets from the TCGA (The Cancer Genome Atlas) database, thereby combining samples from all three subgroups, for an exploratory transcriptome profiling of RCC subgroups.
Materials and Methods: We used FPKM (fragments per kilobase per million) files derived from the ccRCC, pRCC and chRCC cohorts of the TCGA database, representing transcriptomic data of 891 patients. Using principal component analysis, we visualized datasets as t-SNE plot for cluster detection. Clusters were characterized by machine learning, resulting gene signatures were validated by correlation analyses in the TCGA dataset and three external datasets (ICGC RECA-EU, CPTAC-3-Kidney, and GSE157256).
Results: Many RCC samples co-clustered according to histopathology. However, a substantial number of samples clustered independently from histopathologic origin (mixed subgroup)—demonstrating divergence between histopathology and transcriptomic data. Further analyses of mixed subgroup via machine learning revealed a predominant mitochondrial gene signature—a trait previously known for chRCC—across all histopathologic subgroups. Additionally, ccRCC samples from mixed subgroup presented an inverse correlation of mitochondrial and angiogenesis-related genes in the TCGA and in three external validation cohorts. Moreover, mixed subgroup affiliation was associated with a highly significant shorter overall survival for patients with ccRCC—and a highly significant longer overall survival for chRCC patients.
Conclusions: Pan-RCC clustering according to RNA-sequencing data revealed a distinct histology-independent subgroup characterized by strengthened mitochondrial and weakened angiogenesis-related gene signatures. Moreover, affiliation to mixed subgroup went along with a significantly shorter overall survival for ccRCC and a longer overall survival for chRCC patients. Further research could offer a therapy stratification by specifically addressing the mitochondrial metabolism of such tumors and its microenvironment.
Oesophageal involvement in mucous membrane pemphigoid is considered rare, but it may be underdiagnosed. To assess the incidence of oesophageal involvement in a group of patients with newly diagnosed mucous membrane pemphigoid we retrospectively analysed the medical records of 30 consecutive patients with mucous membrane pemphigoid diagnosed between 2006 and 2016 at the Department of Dermatology, University Hospital Würzburg. Twenty-one patients (70%) reported symptoms indicative of oesophageal mucous membrane pemphigoid. Twelve patients (40%) underwent oesophagogastroduodenoscopy, and oesophageal pathology compatible with mucous membrane pemphigoid was endoscopically found in 9 cases (30%). In all patients indirect and direct immunofluorescence were performed. Patients with and without oesophageal involvement did not differ with regard to the results of indirect immunofluorescence on salt-split human skin and monkey oesophagus. Study results demonstrate the necessity of a standardized diagnostic work-up, including adequate tissue samples for direct immunofluorescence, to prevent underdiagnosis of oesophageal mucous membrane pemphigoid.
Interferon alpha (IFNα) is approved for adjuvant treatment of stage III melanoma in Europe and the US. Its clinical efficacy, however, is restricted to a subpopulation of patients while side effects occur in most of treated patients. Thus, the identification of predictive biomarkers would be highly beneficial to improve the benefit to risk ratio. In this regard, STAT3 is important for signaling of the IFNα receptor. Moreover, the STAT3 single-nucleotide polymorphism (SNP) rs4796793 has recently been reported to be associated with IFNα sensitivity in metastatic renal cell carcinoma. To translate this notion to melanoma, we scrutinized the impact of rs4796793 functionally and clinically in this cancer. Interestingly, melanoma cells carrying the minor allele of rs4796793 were the most sensitive to IFNα in vitro. However, we did not detect a correlation between SNP genotype and STAT3 mRNA expression for either melanoma cells or for peripheral blood lymphocytes. Next, we analyzed the impact of rs4796793 on the clinical outcome of 259 stage III melanoma patients of which one-third had received adjuvant IFNα treatment. These analyses did not reveal a significant association between the STAT3 rs4796793 SNP and patients' progression free or overall survival when IFNα treated and untreated patients were compared. In conclusion, STAT3 rs4796793 SNP is no predictive marker for the efficacy of adjuvant IFNα treatment in melanoma patients.

Background:
Dermatologists, paediatricians, and general practitioners are often consulted by worried parents for the evaluation of a cutaneous tumor.
Methods:
Selective literature review.
Results:
Only 1-2% of skin tumors excised in children turn out to be malignant when examined histologically. Warning signs of malignancy include rapid growth, firm consistency, diameter exceeding 3 cm, ulceration, a non-movable mass, and presence in the neonatal period. The more common malignant skin tumors in adults-basal cell carcinoma, cutaneous squamous cell carcinoma, and melanoma-are very rare in childhood. Congenital melanocytic nevi and sebaceous nevi bear a lower malignant potential than previously believed; nevertheless, their excision is often indicated. A Spitz nevus can mimic a melanoma both clinically and histologically. Some benign skin tumors of childhood tend to regress spontaneously within a few years but may cause complications at particular locations and when multiple. For infantile hemangiomas requiring systemic treatment because of imminent obstruction or ulceration, propranolol seems to have a far more favorable risk-benefit ratio than corticosteroids.
Conclusion:
Physicians need specialized knowledge in order to decide whether a skin tumor in a child should be excised, non-surgically treated, or further evaluated, or whether it can be safely left untreated because of the likelihood of spontaneous remission.
Background
Referring to individuals with reactivity to honey bee and Vespula venom in diagnostic tests, the umbrella terms “double sensitization” or “double positivity” cover patients with true clinical double allergy and those allergic to a single venom with asymptomatic sensitization to the other. There is no international consensus on whether immunotherapy regimens should generally include both venoms in double sensitized patients.
Objective
We investigated the long-term outcome of single venom-based immunotherapy with regard to potential risk factors for treatment failure and specifically compared the risk of relapse in mono sensitized and double sensitized patients.
Methods
Re-sting data were obtained from 635 patients who had completed at least 3 years of immunotherapy between 1988 and 2008. The adequate venom for immunotherapy was selected using an algorithm based on clinical details and the results of diagnostic tests.
Results
Of 635 patients, 351 (55.3%) were double sensitized to both venoms. The overall re-exposure rate to Hymenoptera stings during and after immunotherapy was 62.4%; the relapse rate was 7.1% (6.0% in mono sensitized, 7.8% in double sensitized patients). Recurring anaphylaxis was statistically less severe than the index sting reaction (P = 0.004). Double sensitization was not significantly related to relapsing anaphylaxis (P = 0.56), but there was a tendency towards an increased risk of relapse in a subgroup of patients with equal reactivity to both venoms in diagnostic tests (P = 0.15).
Conclusions
Single venom-based immunotherapy over 3 to 5 years effectively and long-lastingly protects the vast majority of both mono sensitized and double sensitized Hymenoptera venom allergic patients. Double venom immunotherapy is indicated in clinically double allergic patients reporting systemic reactions to stings of both Hymenoptera and in those with equal reactivity to both venoms in diagnostic tests who have not reliably identified the culprit stinging insect.
Severe and prolonged liver damage in pityriasis rubra pilaris treated with acitretin: a case report

Acitretin is a systemic retinoid that is used in dermatology for treatment of various inflammatory and especially hyperkeratotic diseases. Elevation of liver enzymes may occur occasionally but normally resolves spontaneously, at the latest after termination of acitretin. However, it can very rarely develop into a life-threatening adverse event including drug-induced liver injury (DILI). A 45-year-old man with classical pityriasis rubra pilaris, a frequently severe, inflammatory skin disease, was started on acitretin. After a seemingly harmless elevation of transaminases, a few weeks after initiation of acitretin, the patient experienced a dramatic course of liver injury with hepatic jaundice though acitretin was stopped immediately. Eventually, laboratory values recovered upon high-dose oral prednisolone therapy. Prescribing physicians should keep in mind that acitretin might induce severe liver injury. Even after termination of acitretin laboratory values should be monitored for a while in order to recognize symptomless but harmful drug-induced liver injury in time.
Die vorliegende Arbeit zeigt eine Möglichkeit auf, die bisher meist erfolglose Chemotherapie des malignen Melanoms zu verbessern: Durch Inhibition des Transkriptionsfaktors NF-kB, der für die Regulation vieler tumorrelevanter Gene verantwortlich ist, konnten die Tumorzellen gegenüber der Wirkung von Zytostatika sensibilisiert werden. Zunächst wurden acht verschiedene Melanomzellen in Bezug auf ihre NF-kB-Aktivität und der Expression NF-kB-regulierter Proteine vergleichen. Es konnte gezeigt werden, dass die Mehrzahl der Melanomzellen über konstitutive Aktivität von NF-κB verfügt. Dabei bestand kein eindeutiger Zusammenhang zwischen der Expression NF-kB-regulierter Proteine und der Aktivität dieses Transkriptionsfaktors im Kern, was komplexe Regulationsmechanismen bei der Transkription und Translation vermuten lässt. Anhand einer ausgewählten Melanomzelllinie konnte gezeigt werden, dass zwei verschiedene NF-kB-Inhibitoren, der Proteasom-Inhibitor Bortezomib und der neue IKK-Inhibitor KINK-1 die Aktivität von NF-kB deutlich hemmen. Beim Vergleich beider NF-kB-Inhibitoren ließen sich unerwartet verschiedene molekulare Wirkungsmechanismen nachweisen: Während Bortezomib konzentrationsabhängig eine sehr starke Induktion von NOXA, eine Induktion von p53 sowie eine Abnahme von Cyclin D1 bewirkte, zeigte KINK-1 seine Effekte vor allem in der Reduktion von Chemokinen wie IL-8 und MCP-1. Passend zur Veränderung der Expression zellzyklus-relevanter Proteine hatte Bortezomib einen stärkeren Effekt auf den Zellzyklus als KINK-1. Beide Inhibitoren wurden mit verschiedenen Zytostatika kombiniert und konnten einerseits die Apoptoseinduktion durch Zytostatika verstärken und andererseits die durch Zytostatika reduzierte Invasion weiter reduzieren. Allerdings zeigte sich bei der Untersuchung tumorrelevanter Chemokine, dass KINK-1 im Gegensatz zu Bortezomib synergistische Effekte mit Camptothecin und Doxorubicin aufweist. Trotz molekularer Unterschiede bewirkten beide NF-kB-Inhibitoren vergleichbare funktionelle Effekte auf zellulärer Ebene. Dies galt auch für ein präklinisches in-vivo-Modell, in dem die experimentelle Lungenmetastasierung von B16F10-Melanomzellen in Mäusen ermittelt wurde: Hier wurden die Mäuse mit Camptothecin, KINK-1 und Bortezomib allein im Vergleich zu den jeweiligen Kombinationen aus Zytostatikum und NF-kB-Inhibitor behandelt. Beide Kombinationen zeigten eine signifikante Reduktion des Lungengewichts im Vergleich zu Camptothecin allein. Diese Arbeit konnte also den Nutzen aus NF-kB-Inhibition in Kombination mit Zytostatika für die hier verwendeten Substanzen bekräftigen und dabei einige molekulare Unterschiede aufdecken.
Merkel cell carcinoma (MCC) is an aggressive neuroendocrine skin cancer that has been associated with the Merkel cell polyomavirus (MCPyV). Indeed, MCC is one of the cancers with the best-established viral carcinogenesis. Despite persistence of the virus in MCC cells and the subsequent expression of viral antigens, the majority of MCC tumors are able to escape the surveillance of the immune system. Therefore the aim of the here presented thesis was to scrutinize immune escape mechanisms operative in MCC. A better understanding of their underlying molecular processes should allow to improve immunotherapeutic treatment strategies for MCC patients. The manuscripts included in this thesis characterize three novel immune evasion strategies of MCC.
I) the epigenetic silencing of the NKG2D ligands MICA and MICB via histone H3 hypoacetylation
II) reduced HLA class I surface expression via epigenetic silencing of the antigen processing machinery (APM)
III) the activation of the PI3K-AKT pathway in a mutation independent manner as potential immune escape strategy
MCC tumors and MCC cell lines were analyzed for their expression of MICA/B, HLA and components of the antigen processing machinery as well as for the activation of the PI3K-AKT pathway in situ and in vitro. These analysis reviled MICA and MICB, as well as HLA class I were not expressed or at least markedly reduced in ~80% of MCCs in situ. The PI3K-AKT pathway, that had only recently been demonstrated to play a significant role in tumor immune escape, was activated in almost 90% of MCCs in situ. To determine the underlying molecular mechanisms of these aberrations well characterized MCC cell lines were further analyzed in vitro. The fact that the PI3K-AKT pathway activation was due to oncogenic mutations in the PIK3CA or AKT1 gene in only 10% of MCCs, suggested an epigenetic regulation of this pathway in MCC. In line with this MICA/B as well as components of the APM were indeed silenced epigenetically via histone hypoacetylation in their respective promoter region. Notably MICA/B and HLA class I expression on the cell surface of MCC cells could be restored after treatment with HDAC inhibitors in combination with the Sp1 inhibitor Mithramycin A in all analyzed MCC cell lines in vitro and in a xenotransplantation mouse model in vivo. Moreover inhibition of HDACs increased immune recognition of MCC cell lines in a MICA/B and HLA class I dependent manner.
Several studies have accumulated evidence that immunotherapy is a promising treatment option for MCC patients due to the exquisite immunogenicity of this malignancy. However, current immunotherapeutic interventions towards solid tumors like MCC have to account for the plentitude of tumor immune escape strategies, in order to increase response rates. The immune escape mechanisms of MCC described in this thesis can be reverted by HDAC inhibition, thus providing the rationale to combine ‘epigenetic priming’ with currently tested immunotherapeutic regimens.
Background:
There is a paucity of studies examining the safety of venom immunotherapy (VIT) in children. We aimed to assess the incidence of anaphylactic side effects during rush VIT in a cohort of pediatric patients and adult controls.
Methods:
72 consecutive cycles of VIT-buildup in 71 children/adolescents aged 7–17 years were retrospectively evaluated and compared to an adult control group (n = 981) with regard to baseline parameters (sex, causative venom, severity of index sting reaction, results of allergy testing, comorbidities) and the incidence of anaphylactic adverse reactions.
Results:
Compared to adults, severe index sting-induced anaphylaxis was significantly less common in children (P = .001). Children were more likely to suffer from bee venom allergy (P < .001) and showed higher levels of bee venom-specific IgE (P = .013), but lower serum tryptase concentrations (P = .014). The overall rate of VIT-induced anaphylactic reactions was higher in children than in adults (6.9% vs 2.5%, P = .046 by univariate analysis). In the final binary logistic regression model, however, only bee VIT (P = .039; odds ratio 2.25; confidence interval 1.04–4.87) and 5-day compared to 3-day buildup protocols (P = .011; odds ratio 2.64; confidence interval 1.25–5.57) were associated with an increased risk of treatment-induced anaphylaxis. All pediatric patients finally reached and tolerated the target maintenance dose of 100 µg.
Conclusions:
The higher anaphylactic reaction rate observed in pediatric patients may be attributed to a greater prevalence of bee venom allergy. VIT-induced anaphylaxis in children is usually mild and does not affect further updosing and maintenance of VIT.
S2k guidelines for the treatment of pemphigus vulgaris/foliaceus and bullous pemphigoid: 2019 update

Background
Correct recognition of risk factors enables individualized management and treatment of venom allergic patients.
Methods
Systematic research and review of current literature regarding the risk of (1) severe sting-induced anaphylaxis, (2) anaphylactic adverse event during venom immunotherapy (VIT), and (3) treatment failure.
Results and discussion
(1) Mastocytosis is the most important risk factor for severe sting-induced anaphylaxis. Hereditary α‑tryptasemia was recently identified as a genetic predictor of severe reactions. Older age is clearly associated with an increased risk; the respective impact of defined cardiovascular comorbidities has yet to be determined. Recent data do not support an aggravation of venom-induced anaphylaxis by intake of β‑blockers or angiotensin-converting enzyme (ACE) inhibitors. A higher risk in men can be attributed to more intensive exposure to stinging insects. (2) Anaphylactic side effects of VIT are most common during the buildup phase, particularly in the course of (ultra-)rush protocols involving a high number of injections and high cumulative daily doses. They are significantly more frequent during honeybee compared to Vespula VIT. Data supporting a negative effect of mastocytosis on the tolerability of VIT are scarce. Older age and cardiovascular medication are not associated with a higher incidence of VIT-induced anaphylaxis. (3) Relapsing anaphylactic reactions to both field and challenge stings are significantly more common during and after honeybee compared to Vespula VIT. Reports of severe field-sting reactions in mastocytosis patients suggest an increased risk of treatment failure which may be overcome by higher maintenance doses and longer duration of VIT.
Merkel cell carcinoma (MCC) is a virally associated cancer characterized by its aggressive behavior and strong immunogenicity. Both viral infection and malignant transformation induce expression of MHC class I chain-related protein (MIC) A and B, which signal stress to cells of the immune system via Natural Killer group 2D (NKG2D) resulting in elimination of target cells. However, despite transformation and the continued presence of virally-encoded proteins, MICs are only expressed in a minority of MCC tumors in situ and are completely absent on MCC cell lines in vitro. This lack of MIC expression was due to epigenetic silencing via MIC promoter hypo-acetylation; indeed, MIC expression was re-induced by pharmacological inhibition of histone deacetylases (HDACs) both in vitro and in vivo. This re-induction of MICs rendered MCC cells more sensitive to immune-mediated lysis. Thus, epigenetic silencing of MICs is an important immune escape mechanism of MCCs.
Ziel dieser Arbeit war es, retrospektiv die Effektivität und Nebenwirkungen einer kombinierten Therapie aus Dapson, oralem Methylprednisolon und topischem Clobetasolpropionat bei Patienten mit einem bullösen Pemphigoid auszuwerten. Der Einsatz von Dapson erfolgte hierbei primär mit dem Ziel, die zur Behandlung des bullösen Pemphigoids notwendige Steroiddosis möglichst schnell senken zu können und so steroidtypische Nebenwirkungen soweit möglich zu vermeiden. Wir konnten zeigen, dass Dapson in Kombination mit systemischen und topischen Kortikosteroiden eine relativ sichere und wirksame Behandlungsstrategie zur Therapie des bullösen Pemphigoids darstellt.
Die vorliegende Arbeit analysiert retrospektiv die funktionellen Beeinträchtigungen und Einschränkungen der Lebensqualität von Kindern und Jugendlichen mit primärer fokaler Hyperhidrose.
Grundlage der vorliegenden Untersuchung bildeten ein selbst entworfener Fragebogen sowie die Auswertung der Ambulanzkarten von 35 Kindern und Jugendlichen mit primärer fokaler Hyperhidrose im Alter von unter 18 Jahren, die sich im Zeitraum von Januar 2000 bis Dezember 2009 in der Hyperhidrose-Sprechstunde der Hautklinik des Universitätsklinikums Würzburg vorstellten. Die vorliegende Arbeit spezifiziert die täglichen Einschränkungen, welche die Kinder und Jugendlichen erleben. Für zwei Drittel der Patienten war das Schwitzen nach Maßgabe der Hyperhidrosis Disease Severity Scale kaum zu ertragen oder unerträglich und beeinträchtige häufig oder immer die täglichen Aktivitäten. Zwei Drittel der Patienten gaben an, derart stark unter der Krankheit zu leiden, dass sie ihre Lebensweise der Krankheit anpassten. Weiter konnte gezeigt werden, dass die Einschränkungen ähnlich gravierend sind wie bei Erwachsenen und anderen Dermatosen in der Kindheit. Die psychische und soziale Entwicklung kann bei einigen Betroffenen nachhaltig beeinträchtigt werden, was sich auch auf das spätere Leben auswirken kann. Diese Erkenntnis ist wichtig, um eine frühzeitige Diagnose und adäquate Behandlung voranzutreiben.
ICIs sind inzwischen integrales Therapiemittel vieler Tumoren, selbst in nicht metastasierten Stadien. Das Management von dabei eventuell entstehenden Nebenwirkungen bleibt wichtiger Bestandteil der Therapie vor allem im fortgeschrittenen Alter. Retrospektive Untersuchungen wie unsere tragen dazu bei, das in vielen klinischen Studien unterrepräsentierte Patientenkollektiv älterer Patienten in den klinischen Alltag sowie in Therapieentscheidungen und -planungen zu integrieren.
Der primäre Studienendpunkt unserer Arbeit unterstützt wichtige Erkenntnisse anderer Studien, dass irAEs insgesamt unter älteren Patienten nicht häufiger auftreten. Zwischen allen drei Altersklassen von ~55, ~70 und ~80 Jahren zeigten sich keine signifikanten Unterschiede im Auftreten von irAEs aller Grade, wobei irAEs Grad III/IV etwas häufiger bei ~80-Jährigen auftraten. In unserem Fall stellten wir fest, dass auftretende irAEs im Alter häufiger behandelt wurden, und dass die Immuntherapie häufiger pausiert oder abgebrochen wurde. Zudem war der Anteil an Therapieabbrüchen unter den älteren Patienten wegen bestimmter Ereignisse wie TRAEs und dem Einsatz von Glukokortikoiden höher als bei jüngeren Patienten.
Die Ergebnisse unserer Studie deuten außerdem darauf hin, dass selbst unter Polypharmazie und Multimorbidität irAEs nicht häufiger bei Älteren auftraten. Ebenso können wir die interessante Beobachtung verzeichnen, dass Patienten mit >5 Medikamenten und gleichzeitig >5 Erkrankungen signifikant mehr irAEs Grad III/IV aufwiesen oder mehr Patienten Glukokortikoide verabreicht bekommen haben. Auch der Anteil an Interventionsbedarf oder Therapieabbruch war hier in allen Altersklassen höher. Es stellt sich die Frage, inwiefern hohes Alter, Komorbidität und Polypharmazie Risikofaktoren für Interventionsbedarf oder Therapieabbruch in der Immuntherapie sind, und ob ihnen eher besondere Gewichtung als Risikofaktor zukommt als dem Alter selbst.
Diese retrospektive Analyse untersucht die Behandlung von Patienten mit Alopecia areata in der Klinik und Poliklinik für Dermatologie, Venerologie und Allergologie der Universitätsklinik Würzburg mit Diphenylcyclopropenon und beleuchtet Faktoren der durch die Behandlung beeinflussten Lebensqualität.
Bei Melanomen des Nagelorgans gilt die operative Entfernung als Therapie der Wahl. Der standardmäßige Eingriff besteht in der Amputation der Phalanx im Interphalangealgelenk oder Metakarpo- bzw. Metatarsophalangealgelenk. Immer mehr Autoren schlagen jedoch ein konservativeres Verfahren in Form einer lokalen Exzision des Tumors mit vollständiger Entfernung des Nagelorgans, aber mit Erhalt des Endglieds vor. In dieser Arbeit wurde ein Kollektiv von 62 Patienten mit der Diagnose „unguales Melanom“, die im Zeitraum von 1988 bis 2012 in der Klinik für Dermatologie, Allergologie und Venerologie des Universitätsklinikums Würzburg behandelt wurden retrospektiv untersucht. Es wurde analysiert, ob das endgliederhaltende Operationsverfahren einer Amputation der Phalanx hinsichtlich des Gesamtüberlebens und des Rezidiv- und Metastasierungsverhaltens des Melanoms ebenbürtig ist und ob das endgliederhaltende Operationsverfahren einer Amputation in funktioneller und ästhetischer Hinsicht überlegen ist.
In dieser Studie werden demographische, diagnostische und therapeutische Faktoren einer an kutanen Sarkomen erkrankten Patientengruppe der Würzburger Hautklinik untersucht. Der prognostische Wert dieser Faktoren wird mit Hilfe verschiedener statistischer Tests und Vergleiche untersucht. Die Studienpopulation umfasst 119 Patienten, die im Zeitraum von 1999 bis 2009 behandelt wurden.
Diese Arbeit umfasst eine retrospektive Analyse von 108 Patienten mit leukozytoklastischer Vaskulitis (LcV), welche an der Universitätshautklinik Würzburg in den Jahren 2001-2007 behandelt wurden. Zunächst wurde eine Auswertung aller Patienten unter demographischen, labordiagnostischen wie auch therapeutischen Gesichtspunkten durchgeführt. Zusätzlich erfolgte eine Analyse von Patienten mit schwerem Krankheitsverlauf in fünf Patientengruppen (hämorrhagisch-nekrotisierende Vaskulitis (n=42), Vaskulitis oberhalb der Gürtellinie (n=62), Nierenbeteiligung (n=36), Rezidiv (n=19), prästationärer Krankheitsverlauf über drei Wochen (n=39)). Ziel dieser Arbeit war, Risikofaktoren für einen schwerwiegenden oder chronischen Verlauf der Erkrankung aufzuzeigen. Zusätzlich wurde ein weiterer Schwerpunkt auf die Analyse möglicher Auslöser einer LcV gelegt. Die Auswertung zeigte am häufigsten Infekte (68,3%) als Ursache einer LcV. Eher selten schienen maligne Erkrankungen (6,7%), Kollagenosen (5,8%) oder Medikamente (6,7%) an der Entwicklung der LcV beteiligt zu sein. In 12,5% der Fälle konnte trotz intensiver Focus-Suche und ausgedehnter Labordiagnostik keine Ursache für die Entstehung einer LcV gefunden werden. Die Ergebnisse widerlegen Angaben älterer Studien, die Medikamente als primären Auslöser einer LcV postulieren. Bei 21,74% der Patienten mit Rezidiv konnte keine Ursache für die LcV gefunden werden, im Vergleich zu 9,26% der Patienten ohne Rezidiv (p=0,075). So konnte gezeigt werden, dass eine intensive Infektfocussuche und deren anschließende Sanierung das Auftreten von Rezidiven der LcV reduziert. Risikofaktoren für einen schwerwiegenden oder chronischen Verlauf einer LcV werden in der Literatur bisher kontrovers diskutiert. In der vorliegenden Studie konnten folgende Korrelationen aufgezeigt werden: Patienten mit nekrotisierender Vaskulitis litten hoch signifikant (p=0,0001) und Patienten mit Nierenbeteiligung signifikant (p=0,016) häufiger an Diabetes mellitus. Zudem war bei Patienten mit systemischer Beteiligung der LcV (p=0,005) und schwerwiegendem Hautbefall (p=0,008) signifikant häufiger IgA im Serum erhöht. Als Risikofaktoren für einen schwerwiegenden Krankheitsverlauf wie für eine systemische Beteiligung der LcV konnten somit folgende Parameter erhoben werden: Diabetes mellitus (RR=1:1,95 (1,17-3,25)) und IgA-Erhöhung im Serum (RR=1:2,11 (1,28-3,48)). Bei Patienten mit chronischem Krankheitsverlauf waren signifikant häufiger B-Symptome (RR=1: 3,19 (1,27-3,19)), der Nachweis von Kryoglobulinen (RR=1: 4,12 (1,65-10,24)), eine Komplement-Erhöhung von C3/C4 (RR=1:4,88 (2,36-10,05)), ein prästationärer Verlauf von über 3 Wochen (RR=1:6,64 (2,37-18,60)) sowie eine Urtikaria-Vaskulitis (RR=1:3,33 (1,44-7,68)) zu beobachten. Die in dieser Arbeit ermittelten Risikofaktoren für einen schwerwiegenden oder chronisch-rezidivierenden Verlauf einer LcV könnten in Zukunft dazu beitragen, früher einen bestimmten Krankheitsverlauf abschätzen zu können und entsprechende Therapieoptionen einzuleiten.
Das Ziel der vorliegenden Arbeit war die retrospektive Analyse von Patienten mit einer mykobakteriellen Hautinfektion, die sich an der Klinik und Poliklinik für Dermatologie, Venerologie und Allergologie des Universitätsklinikums Würzburg oder in der Fachabteilung für Tropenmedizin der Missioklinik Würzburg in dem Zeitraum zwischen 1998 und 2018 vorgestellt haben. Es wurden verschiedene Aspekte wie die Anamnese, der mutmaßliche Infektionsweg, klinische und diagnostische Daten sowie die durchgeführte Therapie untersucht. Es konnten insgesamt 32 Fälle erfasst werden, die in 4 erregerspezifische Gruppen unterteilt wurden. Die Gruppe der Mycobacterium (M.) marinum-Infektionen war mit 18 Fällen am größten, gefolgt von 9 Infektionen mit weiteren nichttuberkulösen Mykobakterien (NTM; M. chelonae, M. avium, M. fortuitum, M. wolinskyi/ mageritense sowie M. haemophilum), 3 Fällen mit dem Erreger M. leprae sowie 2 kutanen M. tuberculosis-Infektionen. Der Anteil immunsupprimierter Patienten mit einer M. marinum-Infektion lag bei 22 %. In unserer Untersuchung stellte der Umgang mit Zierfischaquarien den größten Risikofaktor für eine Infektion mit M. marinum dar. Wie unsere Studie weiterhin zeigte, sollte auch bei knotigen Hautveränderungen in für kutane Mykobakteriosen untypischen Körperarealen, wie z.B. dem Gesicht, eine solche Infektion differentialdiagnostisch erwogen werden. Der kulturelle Nachweis misslang lediglich bei 2 Patienten mit einer M. marinum-Infektion. Die Diagnose konnte in diesen beiden Fällen anhand einer charakteristischen Anamnese, Klinik und Histopathologie gestellt werden. In der vorliegenden Untersuchung erfolgte die Therapie bei allen Patienten mittels einer Antibiose. In der M. marinum-Gruppe erwies sich insbesondere der Wirkstoff Clarithromycin als effektiv. Unter der Gabe von Doxycyclin kam es dagegen in 80 % der Fälle zu einer Therapieumstellung. In beiden Gruppen nichttuberkulöser Mykobakteriosen konnten die Monotherapien im Durchschnitt früher erfolgreich beendet werden als die Kombinationstherapien. Letztere kamen allerdings zum Einsatz bei einer zugrundeliegenden Immunsuppression bzw. bei einem ausgedehnten Hautbefund.
Cutaneous squamous cell carcinoma (cSCC) is a common malignancy of the skin and has an overall favorable outcome, except for patients with an advanced stage of the disease. The efficacy of checkpoint inhibitors (CPI) for advanced cSCC has been demonstrated in recent clinical studies, but data from real-world cohorts and trial-ineligible cSCC patients are limited. We retrospectively investigated patients with advanced cSCC who have been treated with CPI in a first-line setting at eight German skin cancer centers registered within the multicenter registry ADOReg. Clinical outcome parameters including response, progression-free (PFS) and overall survival (OS), time-to-next-treatment (TTNT), and toxicity were analyzed and have been stratified by the individual immune status. Among 39 evaluable patients, the tumor response rate (rwTRR) was 48.6%, the median PFS was 29.0 months, and the median OS was not reached. In addition, 9 patients showed an impaired immune status due to immunosuppressive medication or hematological diseases. Our data demonstrated that CPI also evoked tumor responses among immunocompromised patients (rwTRR: 48.1 vs. 50.0%), although these responses less often resulted in durable remissions. In line with this, the median PFS (11 vs. 40 months, p = 0.059), TTNT (12 months vs. NR, p = 0.016), and OS (29 months vs. NR, p < 0.001) were significantly shorter for this patient cohort. CPI therapy was well tolerated in both subcohorts with 15% discontinuing therapy due to toxicity. Our real-world data show that first-line CPI therapy produced strong and durable responses among patients with advanced cSCC. Immunocompromised patients were less likely to achieve long-term benefit from anti-PD1 treatment, despite similar tumor response rates.
Background
High response rates of metastatic melanoma have been reported upon immune checkpoint inhibition by PD-1 blockade alone or in combination with CTLA-4 inhibitors. However, the majority of patients with a primary resistance to anti-PD-1 monotherapy is also refractory to a subsequent combined checkpoint inhibition. In BRAF wildtype patients with a primary resistance to PD-1 inhibitors, therapeutic options are therefore limited and immune-related adverse events (irAE) have to be taken into consideration when discussing a subsequent immunotherapy.
Case presentation
We report the case of a 68-year-old male patient with metastatic melanoma who experienced an acute renal failure with nephrotic syndrome due to a minimal change disease developing after a single dose of the anti-PD-1 antibody pembrolizumab. A kidney biopsy revealed a podocytopathy without signs of interstitial nephritis. Renal function recovered to almost normal creatinine and total urine protein levels upon treatment with oral steroids and diuretics. Unfortunately, a disease progression (PD, RECIST 1.1) was observed in a CT scan after resolution of the irAE. In a grand round, re-exposure to a PD-1-containing regime was recommended. Consensually, a combined immunotherapy with ipilimumab and nivolumab was initiated. Nephrotoxicity was tolerable during combined immunotherapy and a CT scan of chest and abdomen showed a deep partial remission (RECIST 1.1) after three doses of ipilimumab (3 mg/kg) and nivolumab (1 mg/kg).
Conclusion
This case illustrates that a fulminant response to combined checkpoint inhibition is possible after progression after anti-PD-1 monotherapy and a severe irAE.
Blue nevus is a benign melanocytic lesion, typically asymptomatic and of unknown etiology. Several histologic and clinical variants have been distinguished, the most frequent being common blue nevus, cellular blue nevus, and combined blue nevus. Although melanocytic nevi with a satellite lesion are usually suggestive of locally advanced malignant melanoma, very few cases of blue nevi with satellite lesions have been reported. The diagnosis of common or cellular blue nevi is generally straightforward; however, the presence of structures such as irregular edges or satellitosis are highly suggestive for malignancy, and differential diagnoses such as locally advanced malignant melanoma and malignant blue nevus should be considered. Recurrent blue nevi can display atypical features not seen in the primary lesion, such as pleomorphism and mitotic activity. They usually tend to follow a benign course; however, in some cases, recurrence may indicate malignant transformation. We here report the unique case of a 64-year-old woman with a recurrent cellular blue nevus accompanied by satellite lesions. Such a biological behavior resulting in a clinical presentation as a melanoma-like lesion is a rarity in blue nevus and has not been described before.
The pocket protein (PP) family consists of the three members RB1, p107 and p130 all possessing tumor suppressive properties. Indeed, the PPs jointly control the G1/S transition mainly by inhibiting E2F transcription factors. Notably, several viral oncoproteins are capable of binding and inhibiting PPs. Merkel cell polyomavirus (MCPyV) is considered as etiological factor for Merkel cell carcinoma (MCC) with expression of the viral Large T antigen (LT) harboring an intact PP binding domain being required for proliferation of most MCC cells. Therefore, we analyzed the interaction of MCPyV-LT with the PPs. Co-IP experiments indicate that MCPyV-LT binds potently only to RB1. Moreover, MCPyV-LT knockdown-induced growth arrest in MCC cells can be rescued by knockdown of RB1, but not by p107 or p130 knockdown. Accordingly, cell cycle arrest and E2F target gene repression mediated by the single PPs can only in the case of RB1 be significantly reverted by MCPyV-LT expression. Moreover, data from an MCC patient indicate that loss of RB1 rendered the MCPyV-positive MCC cells LT independent. Thus, our results suggest that RB1 is the dominant tumor suppressor PP in MCC, and that inactivation of RB1 by MCPyV-LT is largely sufficient for its growth supporting function in established MCPyV-positive MCC cells.
Merkel cell carcinoma (MCC) is an aggressive, virus-associated, neuroendocrine tumor of the skin mainly affecting immunocompromised patients. Higher intratumoral infiltration with CD3 and CD8 positive T-cells is associated with a better prognosis, highlighting the relevance of the immune system for MCC development and progression. In this study 21 primary MCCs were stained with immune cell markers including CD3, CD4, CD8, CD68, CD20, and S100. Furthermore, tumor-infiltrating neutrophils, tertiary lymphoid structures and PD-L1 expression were analyzed and correlated with overall and recurrence free survival. All MCCs were Merkel Cell Polyomavirus positive. Overall and recurrence-free survival did not correlate with intra-and peritumoral CD3 and CD8 T-cell infiltration. In addition, no significant association regarding prognosis was found for tumor-associated neutrophils, tumor-associated macrophages or PD-L1 positivity in MCCs. Interestingly, the presence of tertiary lymphoid structures (TLS) in the tumor microenvironment significantly correlated with recurrence-free survival (P=0.025). In addition, TLS were significantly associated with a higher CD8/CD4 ratio in the tumor periphery (P=0.032), but not in the center of the tumor (P > 0.999). These results demonstrate for the first time that TLS, easily assessed in paraffin-embedded tissue in the tumor periphery of MCCs, may be a valuable prognostic factor indicating prolonged recurrence free survival.
Background
Penicillins and other beta-lactam antibiotics are the most common elicitors of allergic drug reaction. However, data on the pattern of clinical reaction types elicited by specific beta-lactams are scarce and inconsistent. We aimed to determine patterns of beta-latam allergy, i.e. the association of a clinical reaction type with a specific beta-lactam antibiotic.
Methods
We retrospectively evaluated data from 800 consecutive patients with suspected beta-lactam hypersensitivity over a period of 11 years in a single German Allergy Center.
Results
beta-lactam hypersensitivity was definitely excluded in 595 patients, immediate-type (presumably IgE-mediated) hypersensitivity was diagnosed in 70 and delayed-type hypersensitivity in 135 cases. Most (59 out of 70, 84.3%) immediate-type anaphylactic reactions were induced by a limited number of cephalosporins. Delayed reactions were regularly caused by an aminopenicillin (127 out of 135, 94.1%) and usually manifested as a measles-like exanthem (117 out of 135, 86.7%). Intradermal testing proved to be the most useful method for diagnosing beta-lactam allergy, but prick testing was already positive in 24 out of 70 patients with immediate-type hypersensitivity (34.3%). Patch testing in addition to intradermal testing did not provide additional information for the diagnosis of delayed-type hypersensitivity. Almost all beta-lactam allergic patients tolerated at least one, usually several alternative substances out of the beta-lactam group.
Conclusions
We identified two patterns of beta-lactam hypersensitivity: aminopenicillin-induced exanthem and anaphylaxis triggered by certain cephalosporins. Intradermal skin testing was the most useful method to detect both IgE-mediated and delayed-type beta-lactam hypersensitivity.
(1) Background: C-X-C Motif Chemokine Receptor 4 (CXCR4) and Fibroblast Activation Protein Alpha (FAP) are promising theranostic targets. However, it is unclear whether CXCR4 and FAP positivity mark distinct microenvironments, especially in solid tumors. (2) Methods: Using Random Forest (RF) analysis, we searched for entity-independent mRNA and microRNA signatures related to CXCR4 and FAP overexpression in our pan-cancer cohort from The Cancer Genome Atlas (TCGA) database — representing n = 9242 specimens from 29 tumor entities. CXCR4- and FAP-positive samples were assessed via StringDB cluster analysis, EnrichR, Metascape, and Gene Set Enrichment Analysis (GSEA). Findings were validated via correlation analyses in n = 1541 tumor samples. TIMER2.0 analyzed the association of CXCR4 / FAP expression and infiltration levels of immune-related cells. (3) Results: We identified entity-independent CXCR4 and FAP gene signatures representative for the majority of solid cancers. While CXCR4 positivity marked an immune-related microenvironment, FAP overexpression highlighted an angiogenesis-associated niche. TIMER2.0 analysis confirmed characteristic infiltration levels of CD8+ cells for CXCR4-positive tumors and endothelial cells for FAP-positive tumors. (4) Conclusions: CXCR4- and FAP-directed PET imaging could provide a non-invasive decision aid for entity-agnostic treatment of microenvironment in solid malignancies. Moreover, this machine learning workflow can easily be transferred towards other theranostic targets.
Various subsets of immune regulatory cells are suggested to influence the outcome of therapeutic antigen-specific anti-tumor vaccinations. We performed an exploratory analysis of a possible correlation of pre-vaccination Th17 cells, MDSCs, and Tregs with both vaccination-induced T-cell responses as well as clinical outcome in metastatic melanoma patients vaccinated with survivin-derived peptides. Notably, we observed dysfunctional Th1 and cytotoxic T cells, i.e. down-regulation of the CD3\(\zeta\)chain (p=0.001) and an impaired IFN\(\gamma\)-production (p=0.001) in patients compared to healthy donors, suggesting an altered activity of immune regulatory cells. Moreover, the frequencies of Th17 cells (p=0.03) and Tregs (p=0.02) were elevated as compared to healthy donors. IL-17-secreting CD4\(^{+}\) T cells displayed an impact on the immunological and clinical effects of vaccination: Patients characterized by high frequencies of Th17 cells at pre-vaccination were more likely to develop survivin-specific T-cell reactivity post-vaccination (p=0.03). Furthermore, the frequency of Th17 (p=0.09) and Th17/IFN\(\gamma\)\(^{+}\) (p=0.19) cells associated with patient survival after vaccination. In summary, our explorative, hypothesis-generating study demonstrated that immune regulatory cells, in particular Th17 cells, play a relevant role for generation of the vaccine-induced anti-tumor immunity in cancer patients, hence warranting further investigation to test for validity as predictive biomarkers.
Background: Rhabdoid melanoma is a rare variant of malignant melanoma with characteristic cytomorphologic features. Due to the potential loss of conventional melanocytic markers, histopathologic diagnosis is often challenging. We hypothesize that immunostaining for PReferentially expressed Antigen in MElanoma (PRAME) might have the potential to uncover the melanocytic origin of these dedifferentiated tumors. Methods: Four cases of rhabdoid primary melanomas were assessed by immunohistochemistry for expression of PRAME and conventional melanocytic markers. Immunohistochemical expression patterns were analyzed in the rhabdoid primaries and, if available, associated metastases. Results: All four cases of rhabdoid primary melanomas showed a strong nuclear positivity for PRAME, while the expression of conventional melanocytic markers S100, MART-1, SOX-10 and HMB-45 was variable between the analyzed cases. Conclusions: In summary, we report four cases of rhabdoid primary melanoma with high to intermediate expression of PRAME despite the partial and variable loss of other melanocytic markers. Hence, PRAME might facilitate the recognition of this highly aggressive entity to avoid misdiagnosis due to histopathologic pitfalls.
Die Migration von Tumorzellen im Bindegewebe erfordert adhäsive Zell-Matrix-Interaktionen, die durch Integrine und andere Adhäsionsmoleküle auf der Zelloberfläche vermittelt werden. In 3DKollagenmatrices benötigen hochinvasive MV3-Melanomzellen überwiegend α2β1-Integrine zur Elongation, Adhäsion an den Kollagenfasern und zur Faserbündelung, sowie zur Kraftgenerierung und Migration. Wir haben untersucht, ob die Migration von Tumorzellen in 3D-Kollagenmatrices vollständig durch die Blockade der Integrinfunktion inhibierbar ist, oder ob es kompensatorische Mechanismen gibt, die zur Migration beitragen. Die β1-Integrinfunktion wurde durch verschiedene Methoden reduziert: a) durchflusszytometrische Sortierung der Zellen in Subgruppen mit niedriger und hoher β1-Integrin-Oberflächenexpression; b) Adhäsionsblockade mit monoklonalem anti β1-Antikörper 4B4 oder Rhodocetin, einem selektiven α2β1-Integrininantagonist; und c) Expression von dominant-negativen Peptiden zur Blockade der Funktion der β1-Integrin-zytoplasmatischen Domäne. Alle β1-Integrin-Interferenzstrategien induzierten einen Übergang der konstitutiv vorhandenen mesenchymalen Migration in einen neuen, amöboiden Migrationstyp (Mesenchymal-Amoeboid Transition, MAT), ähnlich der Migrationsweise von Monozyten oder Lymphozyten. Der Übergang zu amöboider Migration ging einher mit dem Verlust der zellvermittelten Kollagenkontraktion und -reorganisation. Subtotale Inhibition der Integrinfunktion (ca. 50%) durch Antikörper 4B4 ergab eine schnelle (0,3-0,4 >m/min) amöboide Migration, während 90-95%ige Absättigung des β1-Integrin- Epitops zu langsamer amöboider Migration (0,03-0,2 >m/min) führte. Induzierte amöboide Migration verursachte eine gleichmäßige Verteilung der β1-Integrine auf der Zelloberfläche, ein diffuses kortikales Aktin-Zytoskelett, und war mit einer ausgeprägten Formanpassung der Zelle an die Matrixstrukturen verbunden, die von kleinen Filopodien oder Oberflächenblebs getragen wurde. Die Befunde wurden für β1-Integrin-defiziente murine embryonale Fibroblasten (MEF) und murine embryonale Stammzellen (GD25) bestätigt. β1-Integrin-defiziente Fibroblasten zeigten eine schnelle, und GD25 ES-Zellen eine langsame amöboide Migration. Somit erfolgte die amöboide Migration ohne β1-Integrin-vermittelte Zell-Matrix-Interaktionen. Weil keine vollständige Immobilisierung der Zellen erzielt wurde, haben wir alternative Mechanismen von Zell-Matrix-Interaktionen untersucht, die zur Restaktivität der amöboiden Migration beitragen. Als potentielle Kandidaten wurden αv-Integrine, die an denaturiertes Kollagen binden, und Oberflächen-Glycokonjugate getestet. Es wurden keine promigratorischen Funktionen RGDabhängiger Integrine (αv oder β3) mittels zyklischer Arginin-Glycin-Asparaginsäure (cRGD)beobachtet. Um herauszufinden, welche Rolle die Oberfächen-Glycokalyx bei der Zellmigration spielen, wurden verschiedene Methoden angewandt: a) Die an die Proteine gebundenen Glycokonjugate wurden mit Hilfe von N- und O-Glycosidasen von der Oberfläche der lebenden Zellen enzymatisch abgespalten; b) um die Sulfatierung der Glycokonjugate zu verhindern, wurden die Zellen in sulfatfreiem Medium kultiviert. Durch beide Methoden wurde die Bindung von Rutheniumrot an die Zelloberfläche(Glycokalyx) um 60% bzw. die von Heparansulfat der Zelloberfläche um 60% bis 100% reduziert. Nicht die Desulfatierung führte zur Ablösung der Zellen vom Kulturflaschenboden, sondern allein dieBehandlung mit N- und O-Glycosidasen. Die gleichzeitige Behandlung von MV3 Melanomzellen mit N-, O- Glycosidase mit Inhibition der β1-, αvβ3-Integrine führten zur Abrundung der Mehrzahl der Zellen, gefolgt von oszillierender Immobilität (‚Running on the spot’) bzw. sehr langsamer Restmigration (<0,1 >m/min). Dagegen war die Migration der MV3-Zellen nach Kultivierung in sulfatfreiem Medium unverändert. Eine ähnliche Hemmung der Migration erfolgte in β1-/- MEFs nach Glycanverdau. Folglich sind β1-Integrine essentiell für fokalisierte Zell-Matrix-Interaktionen, für die mesenchymale Migration und den Matrixumbau, während amöboide Migration ohne Beteiligung von β1-Integrinen erfolgt, aber durch niedrigaffine, diffuse Zell-Matrix-Interaktionen von Oberflächenglycanen vermittelt wird. Somit ist die Glycokalyx ein alternatives Adhäsionssystem für die integrinunabhängige Zellmigration.
Although the clinical presentations of patients with pityriasis lichenoides et varioliformis acuta (PLEVA) may vary, bullae are not usually part of the clinical spectrum. To date, only two other cases of a bullous variant of PLEVA with evidence of autoantibodies against hemidesmosomal antigens have been reported. The term PLEVA pemphigoides was suggested for this unique clinical, pathological and serological combination of both PLEVA and bullous pemphigoid.
Basal cell carcinoma (BCC) is the most common neoplasm in the Caucasian population. Only a fraction of BCC exhibits pigmentation. Lack of melanocyte colonization has been suggested to be due to p53-inactivating mutations in the BCC cells interfering with the p53-proopiomelanocortin pathway and the production of alpha melanocyte-stimulating hormone in the tumor. To evaluate this, we determined tumor pigmentation as well as expression of melan-A and of p53 in 49 BCC tissues bymeans of immunohistochemistry. As expected, we observed a positive relation between tumor pigmentation and melan-A positive intratumoral melanocytes.Melanocyte colonization and, to a lesser extent, p53 overexpression showed intraindividual heterogeneity in larger tumors. p53 overexpression, which is indicative of p53 mutations, was not correlated to melanocyte colonization of BCC. Sequencing of exon 5–8 of the p53 gene in selected BCC cases revealed that colonization by melanocytes and BCC pigmentation is neither ablated by p53 mutations nor generally present in BCCs with wild-type p53.
Pfadbildung durch invasive Melanomzellen : Matrixdefekte, Zellfragmente und erleichterte Migration

Die metastatische Invasion von Tumorzellen durch die extrazelluläre Matrix von Geweben erfordert aktive Zellmigration sowie häufig auch den Umbau der Gewebestruktur. In dieser Arbeit sollte mittels metastasierender MV3-Melonomzellen in einem 3D-Kollagenmatrixmodell der migrationsassozierte Matrixumbau zellulär und molekular untersucht werden, insbesondere die physikalische Charakterisierung gebildeter Matrixdefekte, die molekulare Identifi kation freigesetzter Zellbestandteile, sowie den Einfluß pfadbildender Zellen auf die Invasion nachfolgender Zellen. Die Daten zeigen, daß MV3-Melanomzellen während der Migration durch ein 3DKollagengewebe komplette Zellfragmente in zurückbleibenden röhrenförmigen Trassen deponieren. Diese beinhalteten Zytoplasma und teils Zytoskelett umgeben von intakter Zellmembran mit integrierten Oberflächenrezeptoren wie β1-Integrinen, nicht jedoch DNA-Material. Der Durchmesser der Fragmente lag überwiegend bei 1-5 μm, selten über 10 μm, entsprechend unspezifisch freigesetzter Zellfragmente, die während der Migration vom Zellhinterende abgeschilftet werden. In einem Sphäroidmodell ließen sich mehrere Invasionsfronten nachweisen, in denen einer ersten pfadbildenden Zelle entlang neu gebildeter Matrixtrassen weitere Zellen den gleichen präformierten Trassen folgten. Die videomikroskopischen Befunde wurden mittels Konfokalmikroskopie bestätigt. Eine erwartete höhere Migrationsgeschwindigkeit der nachfolgenden Zellen in dem präformierten Pfad bestätigte sich jedoch nicht. Somit führt die Invasion von MV3-Melanomzellen zur Ausbildung strukturell umgebauter Matrixtrassen, die aus Matrixdefekt freigesetzten Zellfragmenten und angrenzender Extrazellulärmatrix bestehen und nachfolgenden Zellen als Leitstruktur für eine orientierte Form der Invasion dienen (Kettenwanderung). Diese Befunde beleuchten die Dynamik von Zellarrangements ähnlich dem Invasionsmuster in histopathologischen Tumorproben.
Pathogenic relevance of autoantibodies to type XVII collagen from pemphigoid gestationis patients

Pemphigoid gestationis (PG) and bullous pemphigoid (BP) are subepidermal autoimmune blistering diseases characterized by self-reactive T and B cells specific for the transmembrane hemidesmosomal protein type XVII collagen/BP180. Major T and B cell epitopes are located within the immunodominant 16th non-collagenous domain A (NC16A) of type XVII collagen. It has been suggested that pathogenically relevant autoantibodies also bind to this immunodominant region. The aim of this study was to map the epitopes targeted by blister-inducing human autoantibodies. For this purpose, we used an in vitro model of autoantibody-induced leucocyte-dependent dermal-epidermal separation. In contrast to the majority of patients with BP (7 of 10), preadsorption against a recombinant form of the NC16A region abolished the blister-inducing potential of autoantibodies from all PG patients tested (n=5). Using overlapping synthetic peptides, we demonstrate that PG autoantibodies bind to 2 defined epitopes within the NC16A region (aa 500-514 and aa 511-523). Preadsorption using an affinity matrix containing these two epitopes completely abolished dermal-epidermal separation induced by PG autoantibodies (in 8 of 9 patients). These findings provide new insights into the pathogenesis of pemphigoid diseases and should prove helpful for the development of an antigen-specific immunoadsorption therapy in PG.
Background: Primary cutaneous follicular B-cell lymphoma (PCFBCL) represents an indolent subtype of Non-Hodgkin’s lymphomas, being clinically characterized by slowly growing tumors of the skin and common cutaneous relapses, while only exhibiting a low propensity for systemic dissemination or fatal outcome. Up to now, only few studies have investigated underlying molecular alterations of PCFBCL with respect to somatic mutations. Objectives: Our aim was to gain deeper insight into the pathogenesis of PCFBCL and to delineate discriminatory molecular features of this lymphoma subtype. Methods: We performed hybridization-based panel sequencing of 40 lymphoma-associated genes of 10 cases of well-characterized PCFBCL. In addition, we included two further ambiguous cases of atypical B-cell-rich lymphoid infiltrate/B-cell lymphoma of the skin for which definite subtype attribution had not been possible by routine investigations. Results: In 10 out of 12 analyzed cases, we identified genetic alterations within 15 of the selected 40 target genes. The most frequently detected alterations in PCFBCL affected the TNFRSF14, CREBBP, STAT6 and TP53 genes. Our analysis unrevealed novel mutations of the BCL2 gene in PCFBCL. All patients exhibited an indolent clinical course. Both the included arbitrary cases of atypical B-cell-rich cutaneous infiltrates showed somatic mutations within the FAS gene. As these mutations have previously been designated as subtype-specific recurrent alterations in primary cutaneous marginal zone lymphoma (PCMZL), we finally favored the diagnosis of PCMZL in these two cases based on these molecular findings. Conclusions: To conclude, our molecular data support that PCFBCL shows distinct somatic mutations which may aid to differentiate PCFBCL from pseudo-lymphoma as well as from other indolent and aggressive cutaneous B-cell lymphomas. While the detected genetic alterations of PCFBCL did not turn out to harbor any prognostic value in our cohort, our molecular data may add adjunctive discriminatory features for diagnostic purposes on a molecular level.
Die Therapiemöglichkeiten für Patienten im Melanom Stadium IV sind nach wie vor begrenzt und die Erkrankung nur selten heilbar. Eine mögliche Ziel¬struktur in der Melanom¬therapie der Zukunft könnte das im Melanom häufig genetisch wild¬typisch vorliegende p53 sein.
Für vorliegende Arbeit wurden humane Melanomzelllinien, welche stabil mit einem p53-Reportergenkonstrukt transduziert waren, hinsichtlich ihrer p53-Expression, -Akti-vität und -Akti¬vierbarkeit untersucht. Alle verwendeten Melanom¬zell¬¬¬linien exprimierten p53 un¬ab¬hängig vom p53-Mutations¬status. Drei der sieben untersuchten p53-wild¬typischen Melanomzelllinien zeigten keine oder nur sehr niedrige p53-Reporter¬gen¬aktivität. Die anderen vier p53-wildtypischen Zelllinien dagegen waren durch hohe, mittels p53-Knockdown unterdrückbare Reportergen¬expression gekennzeichnet. Die Proliferation dieser Zellen in Gegenwart von aktivem p53 belegt, dass Melanomzellen eine hohe Toleranz gegenüber diesem Tumor¬suppressor besitzen können. Eine weitere Steige¬rung der p53-Expression und -Aktivität durch die Hemmung von MDM2 (mouse double minute 2) mit der Substanz Nutlin-3a führte in den Zellen mit messbarer p53-Aktivität jedoch zu einem G1-Zell¬zyklusarrest. Dies belegt die prinzipielle Eignung von p53 als mögliche thera¬peutische Zielstruktur. Aufgrund ihrer schlechten Biover¬füg¬barkeit und hohen Toxizität gelten MDM2-Inhibitoren bisher aber als ungeeignet für den klinischen Einsatz.
Die Reduktion hoher therapeutischer Nebenwirkungen könnte durch eine Melanom-spezifische Reaktivierung von p53 gelingen. Eine mögliche negativ-modulierende Wirkung des Mela¬¬nom¬¬markers TRP2 (tyrosinase-related-protein 2) auf p53 wurde im Jahr 2010 von Sendoel et al. nach Unter¬suchungen am Fadenwurm C. elegans vorgeschlagen. TRP2 wird beim metastasierten Melanom in mehr als 80 % der Fälle exprimiert und wäre, handelte es sich beim Melanom um einen weit verbreiteten Regulationsmechanismus, ein interessantes Zielprotein, um die Aktivität von p53 zu steigern. Die dargestellten Ergeb¬nisse zeigen, dass TRP2 zwar in vier von fünf Melanomzelllinien exprimiert wurde, die Unterdrückung der TRP2-Expression jedoch weder spezifischen Einfluss auf die p53-Expression noch auf die p53-Reportergenaktivität zeigte. Auch das veränderte Wachs¬tums¬¬¬verhalten der Zellen nach Unterdrückung von TRP2 mittels drei unterschiedlicher shRNAs konnte im Rescue-Experiment, bei dem TRP nach seinem Knockdown ektop exprimiert wurde, keinem spezifischen Effekt von TRP2 auf die p53-Expression oder p53-Reporteraktivität zugeordnet werden. Auch in der TRP2-negativen Zelllinie führte die ektope TRP2-Expression nicht zu einer verminderten p53-Expression oder -Aktivität. Für das im Gegensatz zu MDM2 deutlich melanomspezifischere TRP2 konnte demensprechend kein sicherer regulatorischer Zusammenhang mit p53 dargestellt werden.
Weitere Untersuchungen müssen zeigen, welche Bedeutung wildtypischem p53 im Melanom zukommt und ob sich weitere mögliche p53-Regulatoren als therapeutische Angriffspunkte eignen.
p53 is a central tumor suppressor protein and its inhibition is believed to be a prerequisite for cancer development. In approximately 50% of all malignancies this is achieved by inactivating mutations in the p53 gene. However, in several cancer entities, including melanoma, p53 mutations are rare. It has been recently proposed that tyrosinase related protein 2 (TRP2), a protein involved in melanin synthesis, may act as suppressor of the p53 pathway in melanoma. To scrutinize this notion we analyzed p53 and TRP2 expression by immunohistochemistry in 172 melanoma tissues and did not find any correlation. Furthermore, we applied three different TRP2 shRNAs to five melanoma cell lines and could not observe a target specific effect of the TRP2 knockdown on either p53 expression nor p53 reporter gene activity. Likewise, ectopic expression of TRP2 in a TRP2 negative melanoma cell line had no impact on p53 expression. In conclusion our data suggest that p53 repression critically controlled by TRP2 is not a general event in melanoma.
Outcome of a de-labelling algorithm compared with results of penicillin (β-lactam) allergy testing

Background
Penicillin allergy labels frequently impede guideline-directed treatment with a penicillin or other β-lactam antibiotics. Despite presumed allergy, targeted questioning may indicate a low probability of sensitization and permit reasonably safe administration of the antibiotic in question. In this study, we evaluated a standardized algorithm aiming to differentiate non-allergic patients from those with true allergic β-lactam hypersensitivity.
Methods
We retrospectively applied a de-labelling algorithm in 800 consecutive patients with suspected β-lactam hypersensitivity. All had undergone complete allergy work-up permitting to definitely exclude or diagnose β-lactam allergy between 2009 and 2019.
Results
In 595 (74.4%) out of 800 cases evaluated, β-lactam allergy could be excluded by negative challenge testing. IgE-mediated anaphylaxis was diagnosed in 70 (8.7%) patients, delayed-type hypersensitivity in 135 (16.9%). In 62 (88.6%) anaphylaxis cases, the algorithm correctly advised to use an alternative antibiotic. Accuracy was higher in patients with moderate to severe anaphylaxis (97.7%) compared to those with a history of mild reactions (73.1%). The algorithm correctly identified 122 (90.4%) patients with proven delayed-type hypersensitivity. It permitted de-labelling in 330 (55.5%) out of 595 patients with diagnostic exclusion of penicillin hypersensitivity, but failed to identify the remaining 265 (44.5%) as low-risk cases.
Conclusions
The algorithm detected 89.8% of cases with penicillin (β-lactam) allergy, sensitivity was optimal for moderate to severe anaphylaxis. Study data justify the implementation of a standardized de-labelling algorithm under close supervision in order to permit guideline-directed treatment and reduce the use of broad-spectrum antibiotics as part of an antibiotic stewardship program.
HINTERGRUND: Verschiedene Therapieoptionen für die orofaziale Granulomatose (OFG) wurden in Fallberichten und kleinen Fallserien beschrieben, randomisierte Studien mit Berücksichtigung von Langzeitverläufen sowie standardisierte Therapieempfehlungen fehlen jedoch.
ZIELSETZUNG: Ziele der aktuellen Auswertung waren 1.) die Charakterisierung klinischer Basisparameter bei einer großen Anzahl von Patientinnen/-en mit OFG; 2.) eine Untersuchung der krankheitsbedingten psychischen Belastung; 3.) die Bewertung aktueller Behandlungsstrategien in Hinblick auf den langfristigen Verlauf; und 4.) die Entwicklung eines Therapiealgorithmus zur Verwendung in einer Standard operating procedure (SOP)
METHODE: Wir werteten retrospektiv 61 Patientinnen/-en mit OFG aus, die zwischen 2004 und 2019 in der Klinik für Dermatologie, Venerologie und Allergologie in Würzburg behandelt worden waren. Die Datenerhebung beinhaltete Geschlecht, Alter bei Erkrankungsbeginn, klinische Manifestationen der OFG, Histologie, Begleiterkrankungen und die jeweils eingesetzte Therapie. Dreiundvierzig Patientinnen/-en, bei denen nach der Erstvorstellung mindestens zwei weitere Kontrolltermine dokumentiert waren, wurden gebeten, anhand eines standardisierten Fragenbogens Auskünfte zu ihrer krankheitsbedingten psychischen Belastung und zum langfristigen Verlauf der OFG zu geben.
ERGEBNISSE: Das mediane Alter bei Erkrankungsbeginn lag bei 45 (Gesamtspanne 7-
77) Jahren. Die Mehrzahl der Patientinnen/-en litt an einer Cheilitis granulomatosa (n=58; 95,1 %); nur 6 (9,8 %) wiesen die komplette Trias eines Melkersson-Rosenthal- Syndroms, bestehend aus Cheilitis granulomatosa, Fazialisparese und Lingua plicata auf. Ein Morbus Crohn war in 9 (14,8 %) Fällen nachzuweisen. Von 23 Patientinnen/- en, die auf den Fragenbogen antworteten, berichteten 16 (69,6 %) über eine relevante (mäßige bis schwere) psychische Beeinträchtigung durch die kosmetischen Auswirkungen der OFG. Deutlich weniger fühlten sich durch Schwierigkeiten beim Essen (n=5, 21,7 %) oder beim Sprechen (n=1; 4,3 %) relevant beeinträchtigt. Fünfundzwanzig (41,0 %) Patientinnen/-en wurden mit Prednisolon behandelt. Die
Mehrheit zeigte unter steroidaler Therapie eine Verbesserung (68,0 %) oder sogar eine vollständige Remission (12,0 %), Rückfälle mit dem Ausschleichen des Steroids waren jedoch häufig. Der am häufigsten steroidsparend eingesetzte Wirkstoff war Sulfasalazin (18 Fälle); das therapeutische Ansprechen war uneinheitlich. Nur 2 Patienten mit assoziiertem Morbus Crohn wurden mit Infliximab behandelt, beide zeigten ein ausgezeichnetes Ansprechen. Das mediane Zeitintervall zwischen der letzten ambulanten Vorstellung bis zur Beantwortung des Fragenbogens betrug 49,5 (0-129) Monate. Zum Zeitpunkt der Datenerfassung befanden sich 12 (52,2 %) von 23 Patientinnen/-en in vollständiger Remission und weitere 10 (43,5 %) berichteten über eine leichte, persistierende Schwellung. Nur 5 (21,7 %) Patientinnen/-en berichteten über Episoden eines aktiven Anschwellens innerhalb der letzten 12 Monate vor der Datenerhebung.
SCHLUSSFOLGERUNGEN: Die OFG betrifft alle Altersgruppen und ist nicht auf Kinder bzw. junge Erwachsene beschränkt. Sie geht mit einer erheblichen psychischen Belastung einher, selbst wenn die objektivierbaren funktionellen Einschränkungen nur gering ausfallen. Systemische Steroide erlauben keine langfristige Krankheitskontrolle. Aufgrund seines relativ günstigen Nebenwirkungsprofils kann Sulfasalazin bei Patientinnen/-en steroidsparend eingesetzt werden, die für eine Behandlung mit TNF- alpha-Inhibitoren nicht in Betracht kommen. Angesichts der insgesamt guten Langzeitergebnisse auch bei unbehandelten Patientinnen/-en kommt in milden bis moderat ausgeprägten Fällen auch eine „Wait-and-Watch“-Strategie in Betracht.
Optimierung des Immunoblot-Nachweises von Autoantikörpern bei Blasen bildenden Autoimmundermatosen

Die bullösen Autoimmundermatosen sind organspezifische Autoimmunkrankheiten, die durch das Auftreten einer Autoimmunantwort gegen Strukturproteine der Haut gekennzeichnet sind. Diese Proteine sind wichtig für den Zell-Zell-Kontakt der Keratinozyten bzw. für die Adhäsion der Epidermis auf der Dermis. Die blasenbildenden Autoimmunkrankheiten werden nach den betroffenen Zielstrukturen eingeteilt. Man unterscheidet vier Hauptgruppen: die Pemphigus- und Phemphigoid-Erkrankungen, die Epidermolysis bullosa acquisita und die Dermatitis herpetiformis Duhring. Entscheidend für die Diagnosestellung von bullösen Autoimmundermatosen sind Klinik, Histologie sowie direkte und indirekte Immunfluoreszenzuntersuchung. Zur exakten Einordnung der verschiedenen bullösen Dermatosen ist die Charakterisierung der Autoantikörper durch immunserologische Tests notwendig. In der vorliegenden Arbeit beschäftigten wir uns mit der Optimierung des Immunoblotnachweises. Wir untersuchten die Seren von 120 Patienten mit bullösen Autoimmundermatosen mit Extrakten kultivierter Keratinozyten. 63 von 78 Patienten mit bullösem Pemphigoid erkannten entweder BP180 oder BP230. 16 von 18 Patienten mit Pemphigoid gestationis reagierten ebenfalls mit BP180 seltener auch mit BP230. Fünf von 6 Patienten mit Pemphigus vulgaris erkannten Desmoglein 3, einer davon zusätzlich Desmoglein 1. Zwei von 2 Patienten mit Pemphigus foliaceus reagierten mit Desmoglein 1. Fünf von 5 Patienten mit paraneoplastischem Pemphigus zeigten eine Reaktivität mit Envo-, Peri- oder Desmoplakin. Drei von 11 Patienten mit Schleimhautpemphigoid reagierten mit Laminin 5, ein Patient mit BP180. Autoantikörper gegen Kollagen Typ VII und Beta4-Integrin konnten mit Extrakten kultivierter Keratinozyten nicht nachgewiesen werden. Deshalb versuchten wir in weiteren Experimenten, den Nachweis dieser beiden Antigene zu optimieren. Zwei von 2 Patienten mit Epidermolysis bullosa acquisita erkannten Kollagen Typ VII in dermalen Extrakten und konzentriertem Überstand von kultivierten Keratinozyten. Autoantikörper gegen Beta4-Integrin konnten mit extrazellulärer Matrix von kultivierten Keratinozyten in 3 von 12 Seren von Patienten mit Schleimhautpemphigoid nachgewiesen werden. Da der Nachweis von Laminin 5 mit Extrakten kultivierter Keratinozyten nicht zufriedenstellend war, wurde auch dieser Immunoblotnachweis weiter optimiert. Wir konnten zeigen, dass die extrazelluläre Matrix kultivierter Keratinozyten ein besser geeignetes Substrat zum Nachweis von Antikörper gegen Laminin 5 ist, vor allem zum Nachweis der IgG-Subklassen. Die Ergebnisse dieser Arbeit zeigen insgesamt, dass das Extrakt kultivierter Keratinozyten den größten Teil der verschiedenen Antigene der bullösen Autoimmundermatosen enthält und somit ein effizientes Substrat für den Immunoblotnachweis darstellt. Der Nachweis von Autoantikörper gegen Laminin 5 ist zwar ebenfalls mit diesem Extrakt möglich, sensitiver ist jedoch der Nachweis unter Verwendung extrazellulärer Matrix. Mit extrazellulärer Matrix gelingt auch der Nachweis von Autoantikörpern gegen Beta4-Integrin. Weiterhin konnten wir zeigen, dass der konzentrierte Überstand von kultivierten Keratinozyten zum Nachweis von Autoantikörpern gegen Kollagen Typ VII eine gute Alternative zum dermalen Extrakt darstellt.
Oncogenic role of an epigenetic reader of m\(^6\)A RNA modification: YTHDF1 in Merkel cell carcinoma

Merkel cell carcinoma is a deadly skin cancer, which in the majority of cases is caused by the Merkel cell polyomavirus (MCPyV). The viral small T antigen is regarded as the dominant oncoprotein expressed in the tumor cells. We used genomic screening of copy number aberrations along with transcriptomic analysis to investigate regions with amplification that harbor differentially expressed genes. We identified YTHDF1, a protein that is a reader of N\(^6\)-methyladenosine (m\(^6\)A) RNA modifications, to have high copy gains and to be highly expressed in Merkel cell carcinoma. Importantly, we identified the presence of m\(^6\)A on small T antigen mRNA suggesting a relation between YTHDF1 amplification and MCPyV gene expression. Interestingly, knockdown of YTHDF1 in Merkel cell carcinoma (MCC) cell lines negatively affected the translation initiation factor eIF3 and reduced proliferation and clonogenic capacity in vitro. Furthermore, analysis of survival data revealed worse overall survival in YTHDF1\(^{high}\) MCC patients compared to YTHDF1\(^{low}\) patients. Our findings indicate a novel oncogenic role of YTHDF1 through m\(^6\)A machinery in the tumorigenesis of MCC.
Two sons of a consanguineous marriage developed biventricular cardiomyopathy. One boy died of severe heart failure at the age of 6 years, the other was transplanted because of severe heart failure at the age of 10 years. In addition, focal palmoplantar keratoderma and woolly hair were apparent in both boys. As similar phenotypes have been described in Naxos disease and Carvajal syndrome, respectively, the genes for plakoglobin (JUP) and desmoplakin (DSP) were screened for mutations using direct genomic sequencing. A novel homozygous 2 bp deletion was identified in an alternatively spliced region of DSP. The deletion 5208_5209delAG led to a frameshift downstream of amino acid 1,736 with a premature truncation of the predominant cardiac isoform DSP-1. This novel homozygous truncating mutation in the isoform-1 specific region of the DSP C-terminus caused Carvajal syndrome comprising severe early-onset heart failure with features of non-compaction cardiomyopathy, woolly hair and an acantholytic form of palmoplantar keratoderma in our patient. Congenital hair abnormality and manifestation of the cutaneous phenotype in toddler age can help to identify children at risk for cardiac death.
Background
It is hypothesized that because of higher mast cell numbers and mediator release, mastocytosis predisposes patients for systemic immediate-type hypersensitivity reactions to certain drugs including non-steroidal anti-inflammatory drugs (NSAID).
Objective
To clarify whether patients with NSAID hypersensitivity show increased basal serum tryptase levels as sign for underlying mast cell disease.
Methods
As part of our allergy work-up, basal serum tryptase levels were determined in all patients with a diagnosis of NSAID hypersensitivity and the severity of the reaction was graded. Patients with confirmed IgE-mediated hymenoptera venom allergy served as a comparison group.
Results
Out of 284 patients with NSAID hypersensitivity, 26 were identified with basal serum tryptase > 10.0 ng/mL (9.2%). In contrast, significantly (P = .004) more hymenoptera venom allergic patients had elevated tryptase > 10.0 ng/mL (83 out of 484; 17.1%). Basal tryptase > 20.0 ng/mL was indicative for severe anaphylaxis only in venom allergic subjects (29 patients; 4x grade 2 and 25x grade 3 anaphylaxis), but not in NSAID hypersensitive patients (6 patients; 4x grade 1, 2x grade 2).
Conclusions
In contrast to hymenoptera venom allergy, NSAID hypersensitivity do not seem to be associated with elevated basal serum tryptase levels and levels > 20 ng/mL were not related to increased severity of the clinical reaction. This suggests that mastocytosis patients may be treated with NSAID without special precautions.
Psoriasis is an immune-mediated systemic inflammatory disease that is not limited to the skin but may be associated with arthritis, cardiovascular diseases, metabolic syndrome including diabetes and obesity and, as identified more recently, non-alcoholic fatty liver disease (NAFLD) that occurs in approximately 50 % of all patients with psoriasis. NAFLD is characterized by accumulation of fat in hepatocytes in the absence of excessive alcohol consumption. Over the last two decades, NAFLD has developed to the most common chronic liver disease with an estimated prevalence of 25 % in the Western population. NAFLD ranges from non-inflammatory or bland hepatic steatosis to inflammation of hepatic tissue (non-alcoholic steatohepatitis, NASH) and consecutive liver fibrosis. It is controversial whether the underlying systemic inflammation of psoriasis is contributing to development of NAFLD or if comorbid diseases such as obesity enhance NAFLD development. Recent findings indicate that cytokine-mediated inflammation through TNFα, interleukin (IL)-6 and IL-17 might be the common link between psoriasis and NAFLD. Considering the shared inflammatory pathways, IL-17 pharmacological blockade, which is already well-established for psoriasis, may be a promising strategy to treat both psoriasis and NAFLD. Therefore, early detection of NAFLD and a better understanding of its pathophysiology in the context of the systemic inflammation in psoriasis is important with regard to individualized treatment approaches.
NFATc1 supports imiquimod-induced skin inflammation by suppressing IL-10 synthesis in B cells

Epicutaneous application of Aldara cream containing the TLR7 agonist imiquimod (IMQ) to mice induces skin inflammation that exhibits many aspects of psoriasis, an inflammatory human skin disease. Here we show that mice depleted of B cells or bearing interleukin (IL)-10-deficient B cells show a fulminant inflammation upon IMQ exposure, whereas ablation of NFATc1 in B cells results in a suppression of Aldara-induced inflammation. In vitro, IMQ induces the proliferation and IL-10 expression by B cells that is blocked by BCR signals inducing NFATc1. By binding to HDAC1, a transcriptional repressor, and to an intronic site of the Il10 gene, NFATc1 suppresses IL-10 expression that dampens the production of tumour necrosis factor-α and IL-17 by T cells. These data indicate a close link between NFATc1 and IL-10 expression in B cells and suggest NFATc1 and, in particular, its inducible short isoform, NFATc1/αA, as a potential target to treat human psoriasis.
The introduction of new therapeutic agents has revolutionized the treatment of metastatic melanoma. The approval of adjuvant anti‐programmed death‐1 monotherapy with nivolumab or pembrolizumab, and dabrafenib plus trametinib has recently set a new landmark in the treatment of stage III melanoma. Now, clinical trials have shown that immune checkpoint blockade can be performed in a neoadjuvant setting, an approach established as a standard therapeutic approach for other tumour entities such as breast cancer. Recent studies suggest that a pathological response achieved by neoadjuvant immunotherapy is associated with long‐term tumour control and that short neoadjuvant application of checkpoint inhibitors may be superior to adjuvant therapy. Most recently, neoadjuvant ipilimumab plus nivolumab in stage III melanoma was reported. With two courses of dose‐optimized ipilimumab (1 mg kg−1) combined with nivolumab (3 mg kg−1), pathological responses were observed in 77% of patients, while only 20% of patients experienced grade 3 or 4 adverse events. However, the neoadjuvant trials employing combined immune checkpoint blockade conducted so far have excluded patients with in transit metastases, a common finding in stage III melanoma. Here we report four patients with in transit metastases or an advanced primary tumour who have been treated with neoadjuvant ipilimumab plus nivolumab according to the OpACIN‐neo trial scheme (arm B). All patients achieved radiological disease control and a pathological response. None of the patients has relapsed so far.
Eine durch Immunglobulin E vermittelte Allergie gegen Medikamente und Substanzen, die während einer Narkose verwendet werden, sowie nicht-allergische Überempfindlichkeitsreaktionen auf Medikamente können für eine narkoseassoziierte Anaphylaxie verantwortlich sein. Da die IgE-vermittelte Anaphylaxie eine potenziell lebensbedrohliche Reaktion ist, ist die Identifizierung des auslösenden Allergens unerlässlich, um ein Wiederauftreten der Anaphylaxie während einer Folgenarkose zu vermeiden.
Ziele dieser retrospektiven Studie waren die Auswertung der Ergebnisse einer standardisierten Allergiediagnostik nach narkoseassoziierter Anaphylaxie und die Identifizierung von Unterschieden bzw. Gemeinsamkeiten zwischen Patienten mit IgE-vermittelter Allergie und Patienten mit nicht-allergischer Überempfindlichkeitsreaktion. Darüber hinaus wurde das Patientenkollektiv dahingegen untersucht, wie häufig es in einer erneuten Narkose zu einem weiteren anaphylaktischen Narkosezwischenfall kam und ob präventive Empfehlungen, die aus den Ergebnissen von Allergietests abgeleitet wurden, bei nachfolgenden Narkosen beachtet wurden.
Pruritus tritt verstärkt bei älteren Menschen auf und ist mit vielen verschiedenen Dermatosen unterschiedlichen Ursprungs vergesellschaftet. Pruritus und ein fortgeschrittenes Lebensalter sind auch charakteristisch für die häufigste blasenbildende Autoimmundermatose, das bullöse Pemphigoid. Im prämonitorischen Stadium treten häufig nur Juckreiz und unspezifische Hautveränderungen auf. Das Prodromalstadium eines bullösen Pemphigoids dauert wenige Wochen bis zu mehreren Jahren.
Ziel dieser Arbeit war es, die pruriginösen Erkrankungen Prurigo simplex subacuta [L28.2], Prurigo nodularis [L28.1], eosinophilenreiche Dermatitis [L30.8] und Prurigoform eines atopischen Ekzems [L20.0] im Hinblick auf das klinische, laborchemische und histologische Bild bei der Erstdiagnose der Erkrankungen auszuwerten. Insbesondere sollte überprüft werden, ob bei der Erstdiagnose typische Autoantikörper einer subepidermalen blasenbildenden Autoimmundermatose (BP180, BP230) nachgewiesen werden konnten und trotz des letzendlich ungewöhnlichen Erscheinungsbildes letztlich ein bullöses Pemphigoid vorgelegen haben könnte.
Es erfolgte eine retrospektive Auswertung der oben genannten pruriginösen Erkrankungen, die über einen Zeitraum von über 10 Jahren in der Klinik für Dermatologie, Venerologie und Allergologie des Universitätsklinikums Würzburg behandelt wurden. Die Patienten wurden gemäß ICD-Kodierung in die vier oben genannten Gruppen unterteilt. Nebst Patientencharakteristika wurden die Parameter direkte Immunfluoreszenz (DIF), indirekte Immunfluoreszenz (IIF), ELISA-Testverfahren, Immunoblot, eosinophile Granulozyten, Gesamt-IgE, histologische Untersuchung, Dermographismus und Blasenbildung ausgewertet.
Es konnten insgesamt 325 Patienten in die Studie eingeschlossen werden, bei denen bei der Erstdiagnose einer pruriginösen Erkankung eine IIF auf der humanen Spalthaut und/oder auf dem Affenösophagus als Substrat veranlasst wurde.
Es konnten bei insgesamt 54 (16,7%) Patienten Autoantikörper gegen IgG oder IgA mittels IIF nachgewiesen werden. Bei 42 (76,4%) Patienten wurde eine weiterführende Diagnostik mittels DIF durchgeführt, die bei 37 (88,1%) Personen als negativ befundet wurde. Bei fünf (11,9%) Patienten konnten Autoantikörper gegen IgG, IgA und IgM nachgewiesen werden. Alle stammten aus der Gruppe mit einer Prurigo simplex subacuta [L28.2]. Bei diesen fünf Patienten wurde zusätzlich noch ein ELISA-Test durchgeführt. Nur bei einem Patienten konnten Autoantikörper gegen BP180 und Desmoglein 1 nachgewiesen werden.

Mit dieser Studie konnte aufgezeigt werden, dass bei Patienten mit den Erkrankungen Prurigo simplex subacuta [L28.2], Prurigo nodularis [L28.1], eosinophilenreiche Dermatitis [L30.8] und Prurigoform eines atopischen Ekzems [L20.0] keine erhöhte Bildung von Autoantikörpern gegen die dermoepidermale Junktionszone stattfindet. Dennoch sollte bei Patienten mit pruriginösen Erkrankungen eine serologische Untersuchung mittels IIF – und im Falle einer Positivität mittels ELISA und ggf. DIF durchgeführt werden, vor allem bei älteren Patienten, bei welchen der Pruritus als führendes Symptom beschrieben wird, um die Diagnose einer bullösen Autoimmundermatose sicher ausschließen zu können. Zudem sollte eine Verlaufskontrolle über mehrere Jahre erfolgen, um die Auswirkung des Pruritus als Trigger auf die Bildung von Autoantikörpern einer bullösen Autoimmundermatose zu verfolgen.
Introduction: Mycosis fungoides, the most common type of cutaneous T-cell lymphoma, can manifest in a variety of clinical and histological forms. Bulla formation is an uncommon finding in mycosis fungoides and only approximately 20 cases have been reported in the literature. Case presentation: We present a case of rapidly progressive mycosis fungoides in a 68-year-old Caucasian man who initially presented with erythematous plaques characterised by blister formation. Conclusion: Although mycosis fungoides bullosa is extremely rare, it has to be regarded as an important clinical subtype of cutaneous T-cell lymphoma. Mycosis fungoides bullosa represents a particularly aggressive form of mycosis fungoides and is associated with a poor prognosis. The rapid disease progression in our patient confirms bulla formation as an adverse prognostic sign in cutaneous T-cell lymphoma.
Common birthmarks can be an indicator of underlying genetic disease but are often overlooked. Mongolian blue spots (dermal melanocytosis) are usually localized and transient, but they can be extensive, permanent, and associated with extracutaneous abnormalities. Co-occurrence with vascular birthmarks defines a subtype of phakomatosis pigmentovascularis, a group of syndromes associated with neurovascular, ophthalmological, overgrowth, and malignant complications. Here, we discover that extensive dermal melanocytosis and phakomatosis pigmentovascularis are associated with activating mutations in GNA11 and GNAQ, genes that encode Ga subunits of heterotrimeric G proteins. The mutations were detected at very low levels in affected tissues but were undetectable in the blood, indicating that these conditions are postzygotic mosaic disorders. In vitro expression of mutant GNA11\(^R183C\) and GNA11\(^Q209L\) in human cell lines demonstrated activation of the downstream p38 MAPK signaling pathway and the p38, JNK, and ERK pathways, respectively. Transgenic mosaic zebrafish models expressing mutant GNA11\(^R183C\) under promoter mitfa developed extensive dermal melanocytosis recapitulating the human phenotype. Phakomatosis pigmentovascularis and extensive dermal melanocytosis are therefore diagnoses in the group of mosaic heterotrimeric G-protein disorders, joining McCune-Albright and Sturge-Weber syndromes. These findings will allow accurate clinical and molecular diagnosis of this subset of common birthmarks, thereby identifying infants at risk for serious complications, and provide novel therapeutic opportunities.
Die Aktivierung der T Zelle bedarf der spezifischen Interaktion zwischen T Zelle und Antigen-präsentierender Zelle unter Ausbildung einer engen Anlagerung beider Zellmembranen („immunologische Synapse“) für Rezeptoren-Interaktionen und konsekutive Signaltransduktion. In dreidimensionaler Kollagenmatrix zeigte sich ein stereotypes, dynamisches Muster bei der Interaktion zwischen CD45RO-positiven humanen T Zellen und antigenpräsentierenden dendritischen Zellen. i) Die Kontaktaufnahme wurde stets über das Leading edge der T Zelle initiiert. ii) Beim dynamischen Kontakt wanderte die T Zelle polarisiert, mit vielen Richtungsänderungen und mit reduzierter Geschwindigkeit auf der DC-Oberfläche, nur unterbrochen von kurzen Stopp- und Abrundungsphasen. Der Uropod der T Zelle stand während der dynamischen Kontakts in kontinuierlicher Verbindung zur DC. iii) Die Loslösung der T Zelle von der DC war ein aktiver Prozess, der durch Interaktion der Vorderfront der T Zelle zu benachbarten Kollagenfasern eingeleitet wurde, gefolgt von der Lösung des Zellkörpers und des Uropods. Alternativ wurden Kontakte durch Uropod-mediierte Retention der T Zelle auf der DC-Oberfläche verlängert. Zur dynamischen molekularen Charakterisierung der Kontaktfläche wurde eine Methode zur Darstellung von Lipid-Rafts an lebenden Zellen in der 3D ECM mit BTRITC etabliert. Die Ergebnisse zeigen ein neues 3-Schritt-Konzept dynamischer und produktiver Interaktionen zwischen T Zelle und DC in vitro. Die assymetrische Kontaktzone impliziert distinkte Funktionen von Vorderfront und Uropod der T Zelle und definiert eine neuartige dynamische Kontaktform für die Signalübertragung zwischen beweglichen Zellen.
Background:
Inguinal lymph node dissection (LND) is a surgical procedure with a high morbidity rate. Variations in surgical procedure, such as sparing of the saphenous vein, have been proposed to reduce surgical morbidity. While sparing of the saphenous vein has shown promising results in earlier studies, data for this procedure in melanoma patients are rare. In this retrospective study, we report 10-year findings on the effects of saphenous vein-sparing LND on surgical morbidity and oncologic outcomes in melanoma patients.
Methods:
A retrospective analysis of melanoma patients receiving inguinal LND in our facility between 2003 and 2013 was performed. Patients were divided into two groups: the saphenous vein resection group and the vein sparing group. Surgical morbidity, including wound infection, lymphatic fistula, severe bleeding, neurological complications, and chronic lymphedema, as well as regional recurrence-free survival were investigated.
Results:
A total of 106 patients were included in this study; of these, the saphenous vein was spared in 41 patients (38.7%). The rate of lymphatic fistula was 51.6 vs. 48.8%, wound infection occurred in 31.3 vs. 24.4%, and patients suffered from chronic lymphedema in 30.0 vs. 26.5% in V. saphena magna resection vs. sparing group. Differences observed, however, were not significant. No difference in regional recurrence-free survival between the two study groups was detected.
Conclusions:
The results of our retrospective analysis could not confirm the promising results reported in earlier studies. Thus, sparing of the saphenous vein appears to be optional.
Simple Summary
In melanoma patients treated with dabrafenib and trametinib, dose reductions and treatment discontinuations related to adverse events (AE) occur frequently. However, the associations between patient characteristics, AE, and exposure are unclear. Our prospective study analyzed serum (hydroxy-)dabrafenib and trametinib exposure and investigated its association with toxicity and patient characteristics. Additionally, the feasibility of at-home sampling of capillary blood was assessed, and a model to convert capillary blood concentrations to serum concentrations was developed. (Hydroxy-)dabrafenib or trametinib exposure was not associated with age, sex, body mass index, or AE. Co-medication with P-glycoprotein inducers was associated with lower trough concentrations of trametinib but not (hydroxy-)dabrafenib. The applicability of the self-sampling of capillary blood was demonstrated. Our conversion model was adequate for estimating serum exposure from micro-samples. The monitoring of dabrafenib and trametinib may be useful for dose modification and can be optimized by at-home sampling and our new conversion model.
Abstract
Patients treated with dabrafenib and trametinib for BRAF\(^{V600}\)-mutant melanoma often experience dose reductions and treatment discontinuations. Current knowledge about the associations between patient characteristics, adverse events (AE), and exposure is inconclusive. Our study included 27 patients (including 18 patients for micro-sampling). Dabrafenib and trametinib exposure was prospectively analyzed, and the relevant patient characteristics and AE were reported. Their association with the observed concentrations and Bayesian estimates of the pharmacokinetic (PK) parameters of (hydroxy-)dabrafenib and trametinib were investigated. Further, the feasibility of at-home sampling of capillary blood was assessed. A population pharmacokinetic (popPK) model-informed conversion model was developed to derive serum PK parameters from self-sampled capillary blood. Results showed that (hydroxy-)dabrafenib or trametinib exposure was not associated with age, sex, body mass index, or toxicity. Co-medication with P-glycoprotein inducers was associated with significantly lower trough concentrations of trametinib (p = 0.027) but not (hydroxy-)dabrafenib. Self-sampling of capillary blood was feasible for use in routine care. Our conversion model was adequate for estimating serum PK parameters from micro-samples. Findings do not support a general recommendation for monitoring dabrafenib and trametinib but suggest that monitoring can facilitate making decisions about dosage adjustments. To this end, micro-sampling and the newly developed conversion model may be useful for estimating precise PK parameters.
Downregulation of miR-221-3p expression in prostate cancer (PCa) predicted overall and cancer-specific survival of high-risk PCa patients. Apart from PCa, miR-221-3p expression levels predicted a response to tyrosine kinase inhibitors (TKI) in clear cell renal cell carcinoma (ccRCC) patients. Since this role of miR-221-3p was explained with a specific targeting of VEGFR2, we examined whether miR-221-3p regulated VEGFR2 in PCa. First, we confirmed VEGFR2/KDR as a target gene of miR-221-3p in PCa cells by applying Luciferase reporter assays and Western blotting experiments. Although VEGFR2 was mainly downregulated in the PCa cohort of the TCGA (The Cancer Genome Atlas) database, VEGFR2 was upregulated in our high-risk PCa cohort (n = 142) and predicted clinical progression. In vitro miR-221-3p acted as an escape mechanism from TKI in PC3 cells, as displayed by proliferation and apoptosis assays. Moreover, we confirmed that Sunitinib induced an interferon-related gene signature in PC3 cells by analyzing external microarray data and by demonstrating a significant upregulation of miR-221-3p/miR-222-3p after Sunitinib exposure. Our findings bear a clinical perspective for high-risk PCa patients with low miR-221-3p levels since this could predict a favorable TKI response. Apart from this therapeutic niche, we identified a partially oncogenic function of miR-221-3p as an escape mechanism from VEGFR2 inhibition.
miR-221 is regarded as an oncogene in many malignancies, and miR-221-mediated resistance towards TRAIL was one of the first oncogenic roles shown for this small noncoding RNA. In contrast, miR-221 is downregulated in prostate cancer (PCa), thereby implying a tumour suppressive function. By using proliferation and apoptosis assays, we show a novel feature of miR-221 in PCa cells: instead of inducing TRAIL resistance, miR-221 sensitized cells towards TRAIL-induced proliferation inhibition and apoptosis induction. Partially responsible for this effect was the interferon-mediated gene signature, which among other things contained an endogenous overexpression of the TRAIL encoding gene TNFSF10. This TRAIL-friendly environment was provoked by downregulation of the established miR-221 target gene SOCS3. Moreover, we introduced PIK3R1 as a target gene of miR-221 in PCa cells. Proliferation assays showed that siRNA-mediated downregulation of SOCS3 and PIK3R1 mimicked the effect of miR-221 on TRAIL sensitivity. Finally, Western blotting experiments confirmed lower amounts of phospho-Akt after siRNA-mediated downregulation of PIK3R1 in PC3 cells. Our results further support the tumour suppressing role of miR-221 in PCa, since it sensitises PCa cells towards TRAIL by regulating the expression of the oncogenes SOCS3 and PIK3R1. Given the TRAIL-inhibiting effect of miR-221 in various cancer entities, our results suggest that the influence of miR-221 on TRAIL-mediated apoptosis is highly context- and entity-dependent.
The extracellular matrix within connective tissues represents a structural scaffold as well as a barrier for motile cells, such as invading tumor cells or passenger leukocytes. It remains unclear how different cell types utilize matrix-degrading enzymes for proteolytic migration strategies and, on the other hand, non-proteolytic strategies to overcome 3D fibrillar matrix networks. To monitor cell migration, a 3D collagen model in vitro or the mouse dermis in vivo were used, in combination with time-lapse video-, confocal- or intravital multiphoton-microscopy, and computer-assisted cell tracking. Expression of proteases, including several MMPs, ADAMs, serine proteases and cathepsins, was shown by flow cytometry, Western blot, zymography, and RT-PCR. Protease activity by migrating HT-1080 fibrosarcoma cells resulting in collagenolysis in situ and generation of tube-like matrix defects was detected by three newly developed techniques:(i) quantitative FITC-release from FITC-labelled collagen, (ii) structural alteration of the pyhsical matrix structure (macroscopically and microscopically), and (iii) the visualization of focal in situ cleavage of individual collagen fibers. The results show that highly invasive ollagenolytic cells utilized a spindle-shaped "mesenchymal" migration strategy, which involved beta1 integrindependent interaction with fibers, coclustering of beta1 integrins and matrix metalloproteinases (MMPs) at fiber bundling sites, and the proteolytic generation of a tube-like matrix-defect by MMPs and additional proteases. In contrast to tumor cells, activated T cells migrated through the collagen fiber network by flexible "amoeboid" crawling including a roundish, elliptoid shape and morphological adaptation along collagen fibers, which was independent of collagenase function and fiber degradation. Abrogation of collagenolysis in tumor cells was achieved by a cocktail of broad-spectrum protease inhibitors at non-toxic conditions blocking collagenolysis by up to 95%. While in T cells protease inhibition induced neither morphodynamic changes nor reduced migration rates, in tumor cells a time-dependent conversion was obtained from proteolytic mesenchymal to non-proteolytic amoeboid migration in collagen lattices in vitro as well as the mouse dermis in vivo monitored by intravital microscopy. Tumor cells vigorously squeezed through matrix gaps and formed constriction rings in regions of narrow space, while the matrix structure remained intact. MMPs were excluded from fiber binding sites and beta1 integrin distribution was non-clustered linear. Besides for fibrosarcoma cells, this mesenchymal-toameboid transition (MAT) was confirmed for epithelial MDA-MB-231 breast carcinoma cells. In conclusion, cells of different origin exhibit significant diversity as well as plasticity of protease function in migration. In tumor cells, MAT could respresent a functionally important cellular and molecular escape pathway in tumor invasion and migration.
A proangiogenic micromilieu is associated with a worse prognosis in systemic lymphoma. Hence, targeting the tumour microenvironment and its vasculature has evolved as a promising novel treatment strategy. The role of tumour neoangiogenesis in cutaneous B-cell lymphoma, however, has not yet been elucidated. Therefore, we examined the expression of vascular endothelial growth factor (VEGF) and its receptors VEGFR-1 and VEGFR-2, as well as microvessel density by immunohistochemistry in paraffin-embedded specimens of different subtypes of primary cutaneous B-cell lymphomas, systemic diffuse large B-cell lymphoma, and cutaneous B-cell pseudolymphoma. Primary cutaneous large B-cell lymphoma (PCLBCL) were characterized by significantly higher intratumoral expression levels of VEGF and its receptors in comparison with the indolent lymphoma subtypes. Moreover, PCLBCL exhibited significantly higher intratumoral microvessel counts. Our study provides evidence that the most aggressive subtype of cutaneous B-cell lymphoma, PCLBCL, is characterized by a proangiogenic micromilieu.
Cutaneous metastatic Crohn’s disease (MCD) is a rare but challenging dermatologic manifestation of Crohn’s disease. It is histologically defined as the presence of non-caseating granulomas at skin sites separated from and non-contiguous to the gastrointestinal tract. Cutaneous metastatic Crohn’s disease should be distinguished from the much more frequent contiguous cutaneous manifestations of Crohn’s disease that present at perianal or, less common, peristomal sites with direct extension from the intestine to the adjacent skin. Versatile clinical presentation and the fact that occurrence can predate the initial diagnosis of Crohn’s disease may lead to misdiagnosis, delayed treatment and underreporting. As case numbers are small and randomized controlled studies on management are lacking, the therapeutic approach remains challenging and is often unsatisfactory. We here performed a systematic literature search identifying 264 published pediatric and adult cases of MCD and additionally report three of our own cases. Our review summarizes clinical characteristics, putative etiopathology, histologic findings, differential diagnoses and treatment options for MCD.
The best characterized polyomavirus family member, i.e., simian virus 40 (SV40), can cause different tumors in hamsters and can transform murine and human cells in vitro. Hence, the SV40 contamination of millions of polio vaccine doses administered from 1955–1963 raised fears that this may cause increased tumor incidence in the vaccinated population. This is, however, not the case. Indeed, up to now, the only polyomavirus family member known to be the most important cause of a specific human tumor entity is Merkel cell polyomavirus (MCPyV) in Merkel cell carcinoma (MCC). MCC is a highly deadly form of skin cancer for which the cellular origin is still uncertain, and which appears as two clinically very similar but molecularly highly different variants. While approximately 80% of cases are found to be associated with MCPyV the remaining MCCs carry a high mutational load. Here, we present an overview of the multitude of molecular functions described for the MCPyV encoded oncoproteins and non-coding RNAs, present the available MCC mouse models and discuss the increasing evidence that both, virus-negative and -positive MCC constitute epithelial tumors.
Merkel cell carcinoma (MCC) is an aggressive skin cancer frequently caused by the Merkel cell polyomavirus (MCPyV), and proliferation of MCPyV-positive MCC tumor cells depends on the expression of a virus-encoded truncated Large T antigen (LT) oncoprotein. Here, we asked in which phases of the cell cycle LT activity is required for MCC cell proliferation. Hence, we generated fusion-proteins of MCPyV-LT and parts of geminin (GMMN) or chromatin licensing and DNA replication factor1 (CDT1). This allowed us to ectopically express an LT, which is degraded either in the G1 or G2 phase of the cell cycle, respectively, in MCC cells with inducible T antigen knockdown. We demonstrate that LT expressed only in G1 is capable of rescuing LT knockdown-induced growth suppression while LT expressed in S and G2/M phases fails to support proliferation of MCC cells. These results suggest that the crucial function of LT, which has been demonstrated to be inactivation of the cellular Retinoblastoma protein 1 (RB1) is only required to initiate S phase entry.
Treatment of recalcitrant cutaneous lupus erythematosus (CLE) is challenging. In situations where conventional treatment approaches fail mepacrine - an antimalarial/antiinfiammatory drug that has fallen into oblivion in the last decades might still be a promising option. We retrospectively analysed medical records of 10 patients with refractory CLE that were treated with mepacrine (100-200 mg/day) as mono- or combination therapy for various time intervals between 2001 and 2013 at the University Hospital Wurzburg. Mepacrine was generally well tolerated. Side effects were mild and usually resolved after reduction or cessation. Over 50% of the patients experienced amelioration of their symptoms despite a previously recalcitrant clinical course. Altogether, our data demonstrate that mepacrine still remains a useful and effective therapeutic option for otherwise treatment-resistant CLE.
Melanozyten (MZ) sind der zweihäufigste epidermale Zelltyp. Das Wachstum und die metabolische Tätigkeit der MZ wird durch humorale Faktoren sowie den direkten Zell-Zell Kontakt kontrolliert. Der schädigende Einfluss der UV-Strahlung, der mit der Entwicklung von epithelialen Tumoren zusammenhängt, wird durch das Pigmentsystem der Haut vermindert. Im Zeitraum von Juni 2006 bis Juni 2007 wurden Präparate mit vorliegenden histologischen und klinischen Befunden von epithelialen Hauttumoren untersucht und mit Melanin-, Melan-A und HMB-45 gefärbt. Hierzu gehören: 40 Basalzellkarzinome (BCC), 20 Plattenepithelkarzinome (PEC), 20 seborrhoische Keratosen (SK), 9 Morbi Bowen (MB), 7 aktinischen Keratosen (AK), und 5 Kerathoakantome (KA). Ziel der Arbeit war die Identifizierung und Charakterisierung der MZ in verschiedenen epithelialen Hauttumoren. Meist waren die Tumorzellen negativ in der Melaninfärbung. Melan-A wurde am stärksten bei SK, BCC und AK exprimiert. Im Gegensatz dazu zeigten sich bei PEC am wenigsten angefärbten Zellen. Die HMB-45-positiv-MZ färbten sich am stärksten bei AK, MB und BCC an. Obwohl HMB-45 bei allen Tumortypen positiv war, war die Auszählung vom HMB-45-positiven-MZ deutlich geringer als bei Melan-A. Es gibt eine Korrelation zwischen MZ und neoplastischen Keratinozyten, jedoch ist noch nicht nachgewiesen, was die MZ dazu stimuliert, zu proliferieren und darüber hinaus zu kolonisieren. Eine Korrelation zwischen der epithelialen Pigmentierung und der Anzahl an MZ zeigte sich nur in gutartigen epithelialen Hauttumoren (SK), während in allen malignen Tumoren keine Korrelation zwischen der MZ-Zahl und eventueller Pigmentierung bestand. Das weist darauf hin, dass hier die Regulation der Pigmentierung zerstört ist.
