Muscle and bone interact via physical forces and secreted osteokines and myokines. Physical forces are generated through gravity, locomotion, exercise, and external devices. Cells sense mechanical strain via adhesion molecules and translate it into biochemical responses, modulating the basic mechanisms of cellular biology such as lineage commitment, tissue formation, and maturation. This may result in the initiation of bone formation, muscle hypertrophy, and the enhanced production of extracellular matrix constituents, adhesion molecules, and cytoskeletal elements. Bone and muscle mass, resistance to strain, and the stiffness of matrix, cells, and tissues are enhanced, influencing fracture resistance and muscle power. This propagates a dynamic and continuous reciprocity of physicochemical interaction. Secreted growth and differentiation factors are important effectors of mutual interaction. The acute effects of exercise induce the secretion of exosomes with cargo molecules that are capable of mediating the endocrine effects between muscle, bone, and the organism. Long-term changes induce adaptations of the respective tissue secretome that maintain adequate homeostatic conditions. Lessons from unloading, microgravity, and disuse teach us that gratuitous tissue is removed or reorganized while immobility and inflammation trigger muscle and bone marrow fatty infiltration and propagate degenerative diseases such as sarcopenia and osteoporosis. Ongoing research will certainly find new therapeutic targets for prevention and treatment.
Akute Patellaluxationen sind häufige Kniegelenksverletzungen. Sie gehen oft mit einer Verletzung des medialen patellofemoralen Ligaments (MPFL) einher, was zu rezidivierenden Patellaluxationen und patellofemoraler Instabilität führen kann. Ein möglicher Therapieansatz ist die Rekonstruktion des MPFL.
Zahlreiche Operationstechniken sind hierzu in der Literatur beschrieben, darunter Fixationen mit Interferenzschrauben, Bohrkanälen, Nahtankern oder Nahtfixationen am Knochen oder Weichgewebe. Sie werden meist mit einem autologen Sehnentransplantats durchgeführt und unterscheiden sich hauptsächlich in der Art dessen patellarer Fixation.
Meta-Analysen zeigen nach wie vor hohe Komplikationsraten und teilweise wenig zufriedenstellende Ergebnisse etablierter MPFL-Rekonstruktionen.
Ein vielversprechender Ansatz zur Senkung der Komplikationsraten sind patellare Fixationsmethoden, die auf das Einbringen von Fremdmaterial und Bohrungen durch die Patella verzichten. Solche implantat- und/oder bohrkanalfreien Fixationsmethoden sind bereits in der Literatur beschrieben, bis jetzt existieren jedoch nur wenige Studien zu deren biomechanischen Eigenschaften.
Ziel dieser Arbeit war der Vergleich biomechanischer Eigenschaften von fünf unterschiedlichen implantatfreien patellaren Fixationsmethoden beim MPFL- Ersatz mit dem nativen MPFL und mit nativen Extensorensehnen. Dazu wurde ein porcines Modell benutzt. Die Hypothese dieser Studie war, dass die patellare Weichteilfixation eines autologen Sehnentransplantats ohne Implantate oder Bohrkanal eine vergleichbare primäre Ausrissfestigkeit wie das native MPFL zeigt.
60 Patellae und Extensorensehnen wurden aus porcinen Hinterläufen extrahiert und randomisiert auf 6 Versuchsgruppen aufgeteilt (n=10). In den Gruppen 1 und 2 wurden patellare Weichteilfixationen getestet. In den Gruppen 3 und 4 wurden Bohrkanalfixationen mit unterschiedlichem Nahtmaterial durchgeführt (Gruppe 3: resorbierbarer Faden der Stärke USP 0, Gruppe 4: nicht-resorbierbarer Faden der Stärke USP 3). In Gruppe 5 erfolgte die patellare Fixation mittels V-förmigen Bohrkanal mit Durchmesser von 4,5 mm. In der sechsten Versuchsgruppe wurde zum Vergleich das native Gewebe getestet. In einer weiteren Versuchsgruppe wurden native porcine Extensorensehnen biomechanisch untersucht.
Die biomechanische Testung gliederte sich in mehrere Abschnitte. Zunächst erfolgte eine Präkonditionierung mit 10 Setzzyklen mit einem Kraftintervall von 5 - 20 N. Es folgte eine zyklische Testphase von 1000 Messzyklen mit einem Kraftintervall von 5 - 50 N. Im Anschluss wurde die maximale Kraft bis zum Versagen angewendet. Dabei wurden die Parameter maximale Ausrisskraft, Yield Load, Steifigkeit, Elongation nach 1000 Zyklen sowie die Versagensart untersucht. In der Versuchsgruppe des nativen Gewebes wurde direkt die maximale Kraft bis zum Versagen angewendet.
Die höchsten maximalen Ausrisskräfte wurden in Gruppen 1 und 2 gemessen (321,8 ± 53,5 N und 242,0 ± 57,4 N) und waren mit dem nativen Gewebe vergleichbar (252,1 ± 129,3 N, p > 0,05). Ein signifikant niedrigerer Yield Load im Vergleich zum nativen Gewebe zeigte sich in den Gruppen 3 (p < 0,01) und 4 (p = 0,01). Während der zyklischen Belastung war die Elongation der Präparate in Gruppe 3 signifikant höher im Vergleich zu den anderen Gruppen (5,3 ± 1 mm, p < 0,05). Alle Gruppen wiesen ähnliche Steifigkeiten wie das native Gewebe vor (p > 0,05). Die getesteten Fixationsmethoden zeigten signifikante Unterschiede in ihren biomechanischen Eigenschaften (p < 0,05). Insbesondere die Weichteilfixationen in Gruppen 1 und 2 wiesen adäquate biomechanische Eigenschaften im Vergleich zum nativen Gewebe vor.
Durch die Kombination aus geringerer Invasivität, Verzicht auf das Einbringen von Fremdkörpern, einfacher Anwendbarkeit und guter Primärstabilität sind patellare Weichteilfixationen vielversprechende Alternativen bei der MPFL-Rekonstruktion.

Introduction/Background
Surfing, wind surfing and kite surfing enjoy a growing popularity with a large number of athletes worldwide. The aim of this study was to identify and compare the injury profiles and compare the injury profiles of these three extreme water sports.
Materials and Methods
These data for this retrospective cohort study were collected through an online standardised questionnaire during the 2017–18 season. The questionnaire included questions about anthropometry, skill level, injury diagnosis, injury mechanism, environmental conditions and training regimes.
Results
The 626 athletes included reported 2584 injuries. On average, each athlete sustained 4.12 injuries during the season. The most frequent injury location was in the lower extremity, in particular the foot, with 49 (16.4%) injuries in surfing, 344 (18.3%) in wind surfing and 79 (19.7%) in kite surfing. Surfing demonstrated a particularly high rate of head injuries (n = 37; 12.4%). Other frequent injury types were skin lesions (up to 42.1%) and contusions (up to 40.5%). The most common injury across all surfing sports was skin lesions of the foot (wind surfing: 11.7%; kite surfing: 13.2%; surfing: 12.7%). In surfing, skin lesions of the head were frequently observed (n = 24; 8.0%). In surfing, a ‘too large wave’ (n = 18; 24.7%) was main cause of the injury, while in wind surfing (n = 189; 34.5%) and kite surfing (n = 65; 36.7%) ‘own incompetence’ led to the most injuries.
Conclusion
This unique study compares injury epidemiology and mechanism in the three most popular surfing sports: wind surfing, kite surfing and surfing. Overall, injuries were sustained mainly in the lower extremity, while surfing also demonstrated a high rate of head injuries.
Diagnosis of chronic infection at total hip arthroplasty revision is a question of definition

Purpose. Contradicting definitions of periprosthetic joint infection (PJI) are in use. Joint aspiration is performed before total hip arthroplasty (THA) revision. This study investigated the influence of PJI definition on PJI prevalence at THA revision. Test quality of prerevision aspiration was evaluated for the different PJI definitions. Methods. 256 THA revisions were retrospectively classified to be infected or not infected. Classification was performed according to the 4 different definitions proposed by the Musculoskeletal Infection Society (MSIS), the Infectious Diseases Society of America (IDSA), the International Consensus Meeting (ICM), and the European Bone and Joint Infection Society (EBJIS). Only chronic PJIs were included. Results. PJI prevalence at revision significantly correlated with the applied PJI definition (p=0.01, Cramer's V=0.093). PJI prevalence was 20.7% for the MSIS, 25.4% for the ICM, 28.1% for the IDSA, and 32.0% for the EBJIS definition. For synovial fluid white blood cell count, the best ROC-AUC for predicting PJI was 0.953 in combination with the MSIS definition. Conclusion. PJI definition significantly influences the rate of diagnosed PJIs at THA revision. Synovial fluid white blood cell count is a reliable means to rule out PJI. In cases with a borderline high synovial white blood cell count before THA revision as the only sign of chronic PJI, an extended diagnostic work-up should be considered.
To date, no consensus exists regarding the best surgical management of isolated, micro-traumatic long thoracic nerve (LTN) paresis. Our hypothesis was that a combined decompression of the LTN at two potential locations for entrapment would be effective in the management of dynamic LTN paresis. We report on twelve patients with isolated LTN parersis, with tenderness at two entrapment sites, who underwent bifocal LTN decompression after undergoing unsuccessful conservative treatment for at least 6 months; all patients had preoperative electrodiagnostic studies that confirmed the paresis and ruled out peripheral neuritis. Clinical and electrical improvements were observed in eight patients (67%) regarding shoulder flexion, shoulder abduction, and Quick-DASH scores. Four patients (33%) did not improve after surgery. The results corroborate our hypothesis that a bifocal LTN decompression can be an effective and reliable therapeutic option in more than half of a very selective patient population suffering from serratus anterior muscle deficiency.
Background
To identify injury patterns and mechanisms in professional men’s basketball by means of video match analysis.
Methods
In Germany, injuries are registered with the statutory accident insurance for professional athletes (VBG) by clubs or club physicians as part of occupational accident reporting. Moderate and severe injuries (absence of > 7 days) sustained during basketball competition in one of four seasons (2014–2017 and 2018–2019) in the first or second national men’s league in Germany were prospectively analyzed using a newly developed standardized observation form. Season 2017–2018 was excluded because of missing video material.
Results
Video analysis included 175 (53%) of 329 moderate and severe match injuries. Contact patterns categorized according to the different body sites yielded eight groups of typical injury patterns: one each for the head, shoulders, and ankles, two for the thighs, and three for the knees. Injuries to the head (92%), ankles (76%), shoulders (70%), knees (47%), and thighs (32%) were mainly caused by direct contact. The injury proportion of foul play was 19%. Most injuries (61%) occurred in the central zone below the basket. More injuries occurred during the second (OR 1.8, p = 0.018) and fourth quarter (OR 1.8, p = 0.022) than during the first and third quarter of the match.
Conclusion
The eight identified injury patterns differed substantially in their mechanisms. Moderate and severe match injuries to the head, shoulders, knees, and ankles were mainly caused by collision with opponents and teammates. Thus, stricter rule enforcement is unlikely to facilitate safer match play.
The fate and behavior of bone marrow mesenchymal stem/stromal cells (BM-MSC) is bidirectionally influenced by their microenvironment, the stem cell niche, where a magnitude of biochemical and physical cues communicate in an extremely orchestrated way. It is known that simplified 2D in vitro systems for BM-MSC culture do not represent their naïve physiological environment. Here, we developed four different 2D cell-based decellularized matrices (dECM) and a 3D decellularized human trabecular-bone scaffold (dBone) to evaluate BM-MSC behavior. The obtained cell-derived matrices provided a reliable tool for cell shape-based analyses of typical features associated with osteogenic differentiation at high-throughput level. On the other hand, exploratory proteomics analysis identified native bone-specific proteins selectively expressed in dBone but not in dECM models. Together with its architectural complexity, the physico-chemical properties of dBone triggered the upregulation of stemness associated genes and niche-related protein expression, proving in vitro conservation of the naïve features of BM-MSC.
Approaches to mimic the complexity of the skeletal mesenchymal stem/stromal cell niche in vitro

Mesenchymal stem/stromal cells (MSCs) are an essential element of most modern tissue engineering and regenerative medicine approaches due to their multipotency and immunoregulatory functions. Despite the prospective value of MSCs for the clinics, the stem cells community is questioning their developmental origin, in vivo localization, identification, and regenerative potential after several years of far-reaching research in the field. Although several major progresses have been made in mimicking the complexity of the MSC niche in vitro, there is need for comprehensive studies of fundamental mechanisms triggered by microenvironmental cues before moving to regenerative medicine cell therapy applications. The present comprehensive review extensively discusses the microenvironmental cues that influence MSC phenotype and function in health and disease – including cellular, chemical and physical interactions. The most recent and relevant illustrative examples of novel bioengineering approaches to mimic biological, chemical, and mechanical microenvironmental signals present in the native MSC niche are summarized, with special emphasis on the forefront techniques to achieve bio-chemical complexity and dynamic cultures. In particular, the skeletal MSC niche and applications focusing on the bone regenerative potential of MSC are addressed. The aim of the review was to recognize the limitations of the current MSC niche in vitro models and to identify potential opportunities to fill the bridge between fundamental science and clinical application of MSCs.
Purpose
The goal of our study was to conduct an online survey that highlights patterns of practice during total hip arthroplasty (THA).
Methods
The survey was conducted in June and August 2020. Three hundred thirteen members of the German Society for Endoprosthesis participated in the survey.
Results
The anterolateral approach is by far the most popular approach used for primary total hip arthroplasty, followed by the anterior approach during minimally invasive (55% for the anterolateral and 29% for the anterior) and regular surgery (52% for the anterolateral and 20% for the anterior). Two-thirds of the orthopaedic surgeons do not use drainages during THA. Moreover, 80% of the survey participants routinely apply tranexamic acid during surgery. Surgeons who perform minimally invasive surgery for THA use more frequently fast-track-concepts for post-operative rehabilitation. According to the interviewees, the application of fast-track-concepts leads to reduced periods of hospital stay after THA.
Conclusion
Our data demonstrate that patterns of practice during THA in Germany are in line with the evidence provided by current literature. This study can be seen as a stimulus to conduct similar surveys in other countries in order to promote minimally invasive surgery for THA.
Summary
There is limited understanding of how asfotase alfa affects mineral metabolism and bone turnover in adults with pediatric-onset hypophosphatasia. This study showed that adults with hypophosphatasia treated with asfotase alfa experienced significant changes in biochemical markers of bone and mineral metabolism, possibly reflecting enhanced bone remodeling of previously osteomalacic bone.
Introduction
Hypophosphatasia (HPP), due to a tissue nonspecific alkaline phosphatase (TNSALP) deficiency, can cause impaired bone mineralization and turnover. Although HPP may be treated with asfotase alfa, an enzyme replacement therapy, limited data are available on how treatment with asfotase alfa affects mineral metabolism and bone turnover in adults with HPP.
Methods
ALP substrates, bone turnover and mineral metabolism markers, and bone mineral density (BMD) data from EmPATHY, a single-center, observational study of adults (≥ 18 years) with pediatric-onset HPP treated with asfotase alfa (NCT03418389), were collected during routine clinical care and analyzed from baseline through 24 months of treatment.
Results
Data from 21 patients showed significantly increased ALP activity and reduced urine phosphoethanolamine (PEA)/creatinine (Cr) ratios after baseline through 24 months of asfotase alfa treatment. There were significant transient increases in parathyroid hormone 1-84 (PTH), osteocalcin, and procollagen type 1 N-propeptide (P1NP) levels at 3 and 6 months and in tartrate-resistant acid phosphatase 5b (TRAP5b) levels at 3 months, with a significant decrease in N-terminal telopeptide of type 1 collagen (NTX) levels at 24 months. Lumbar spine BMD T scores continuously increased during treatment.
Conclusion
Significant changes in bone turnover and mineral metabolism markers after asfotase alfa treatment suggest that treatment-mediated mineralization may enable remodeling and bone turnover on previously unmineralized surfaces. Urine PEA/Cr ratios may be a useful parameter in monitoring treatment during routine care.
Purpose
The present study determined the postoperative phenotypes after unrestricted calipered kinematically aligned (KA) total knee arthroplasty (TKA), whether any phenotypes were associated with reoperation, implant revision, and lower outcome scores at 4 years, and whether the proportion of TKAs within each phenotype was comparable to those of the nonarthritic contralateral limb.
Methods
From 1117 consecutive primary TKAs treated by one surgeon with unrestricted calipered KA, an observer identified all patients (N = 198) that otherwise had normal paired femora and tibiae on a long-leg CT scanogram. In both legs, the distal femur–mechanical axis angle (FMA), proximal tibia–mechanical axis angle (TMA), and the hip–knee–ankle angle (HKA) were measured. Each alignment angle was assigned to one of Hirschmann’s five FMA, five TMA, and seven HKA phenotype categories.
Results
Three TKAs (1.5%) underwent reoperation for anterior knee pain or patellofemoral instability in the subgroup of patients with the more valgus phenotypes. There were no implant revisions for component loosening, wear, or tibiofemoral instability. The median Forgotten Joint Score (FJS) was similar between phenotypes. The median Oxford Knee Score (OKS) was similar between the TMA and HKA phenotypes and greatest in the most varus FMA phenotype. The phenotype proportions after calipered KA TKA were comparable to the contralateral leg.
Conclusion
Unrestricted calipered KA’s restoration of the wide range of phenotypes did not result in implant revision or poor FJS and OKS scores at a mean follow-up of 4 years. The few reoperated patients had a more valgus setting of the prosthetic trochlea than recommended for mechanical alignment. Designing a femoral component specifically for KA that restores patellofemoral kinematics with all phenotypes, especially the more valgus ones, is a strategy for reducing reoperation risk.
Wir stellen den seltenen Fall einer Insuffizienzfraktur der Klavikula nach Implantation einer inversen Schulterendoprothese (RSA) vor. Als Ursache solcher Frakturen wird eine vermehrte Zugbelastung durch den Musculus deltoideus nach RSA diskutiert. In den wenigen verfügbaren Fallberichten zeigten die betroffenen Patienten deutliche Funktionseinschränkungen. Die Versorgung erfolgte im vorliegenden Fall mit Plattenosteosynthese. Trotz intraoperativ gutem Korrekturergebnis kam es im Verlauf ohne Trauma zum Osteosyntheseversagen mit weiterer Dislokation der Fraktur.
Background
Aging is associated with progressive loss of musculoskeletal performance. Exercise interventions can improve physical function in the elderly but there is a paucity of comparative assessments in order to understand what specific goals can be achieved particularly with less demanding exercise interventions readily accessible for untrained men.
Methods
Prospective randomized, controlled, single center exploratory trial to compare four distinct exercise interventions, i.e. Resistance Training (RT), Whole Body Vibration Exercise (WBV), Qi Gong (QG) and wearing a Spinal orthosis (SO) for 6 months in men at risk for osteoporosis aged 65–90 years. Primary endpoint was change in isometric one repetition maximum force trunk strength for extension (TSE) and flexion (TSF) compared to baseline, secondary endpoints covered key parameters of geriatric functional assessment, including Handgrip Strength (HS), Chair-Rise-Test (CRT), Usual Gait Speed (UGS) and Timed-Up-and-Go (TUG).
Results
Altogether 47 men (mean age 77 ±6.1 years) were randomized to RT, (n = 11) WBV (n = 13), QG (n = 10) and SO(n = 13). RT, defined as reference exercise intervention, lead to significant improvements for TSE (p = 0.009) and TSF (p = 0.013) and was significantly superior in the between-group analysis for TSE (p = 0.038). Vibration exercise caused sign. Improvements in TSE (p = 0.014) and CRT (p = 0.005), the Spinal orthosis improved CRT (p = 0.003) and Gait Speed (p = 0.027), while the QG intervention did not attain any sig. Developments.
Subgroup analyses revealed most pronounced musculoskeletal progress in vulnerable patients (age ≥ 80 years, pre-sarcopenia, multimorbidity ≥3chronic diseases). Irrespective of the type of exercise, participants ≥80 years experienced significant gains in TSE (p = 0.029) and CRT (p = 0.017). Presarcopenic subjects (Skeletal muscle Index (SMI) ≤10.75 kg/m2) improved in TSE (p = 0.003), CRT (p = 0.001) and UGS (p = 0.016). Multimorbid participants achieved sig. Gains in TSE (p < 0.001), TSF (p = 0.002), UGS (p = 0.036) and HS (p = 0.046).
Conclusions
In this exploratory trial we found that simple exercise interventions are feasible in elderly men eliciting specific benefits, i.e. improvements are attained in those tasks addressed with the respective exercise modality. While targeted resistance training is superior in increasing TSE, alternative simple exercise interventions also appear to elicit beneficial effects, even in vulnerable patients, i.e. those with low muscle mass, above 80 years of age or multimorbidity.

In vitro evaluation of antibacterial efficacy of vancomycin-loaded suture tapes and cerclage wires

Usage of implants containing antibiotic agents has been a common strategy to prevent implant related infections in orthopedic surgery. Unfortunately, most implants with microbial repellent properties are characterized by accessibility limitations during daily clinical practice. Aim of this in vitro study was to investigate whether suture tapes and cerclage wires, which were treated with vancomycin, show a sustainable antibacterial activity. For this purpose, we used 24 stainless steel wire cerclages and 24 ultra-high molecular weight polyethylene and polyester suture tape test bodies. The test bodies were incubated for 30 min. in 100 mg/ml vancomycin solution or equivalent volumes of 0.9% NaCl. After measuring the initial solution uptake of the test bodies, antibacterial efficacy via agar diffusion test with Staphylococcus aureus and vancomycin elution tests were performed 1, 2, 3, and 6 days after incubation. Vancomycin-loaded tapes as well as vancomycin-loaded cerclage wires demonstrated increased bacterial growth inhibition when compared to NaCl-treated controls. Vancomycin-loaded tapes showed an additional twofold and eightfold increase of bacterial growth inhibition compared to vancomycin-loaded wires at day 1 and 2, respectively. Elution tests at day 1 revealed high levels of vancomycin concentration in vancomycin loaded tapes and wires. Additionally, the concentration in vancomycin loaded tapes was 14-fold higher when compared to vancomycin loaded wires. Incubating suture tapes and cerclage wires in vancomycin solution showed a good short-term antibacterial activity compared to controls. Considering the ease of vancomycin application on suture tapes or wires, our method could represent an attractive therapeutic strategy in biofilm prevention in orthopedic surgery.
Metabolic glycoengineering enables a directed modification of cell surfaces by introducing target molecules to surface proteins displaying new features. Biochemical pathways involving glycans differ in dependence on the cell type; therefore, this technique should be tailored for the best results. We characterized metabolic glycoengineering in telomerase-immortalized human mesenchymal stromal cells (hMSC-TERT) as a model for primary hMSC, to investigate its applicability in TERT-modified cell lines. The metabolic incorporation of N-azidoacetylmannosamine (Ac\(_4\)ManNAz) and N-alkyneacetylmannosamine (Ac\(_4\)ManNAl) into the glycocalyx as a first step in the glycoengineering process revealed no adverse effects on cell viability or gene expression, and the in vitro multipotency (osteogenic and adipogenic differentiation potential) was maintained under these adapted culture conditions. In the second step, glycoengineered cells were modified with fluorescent dyes using Cu-mediated click chemistry. In these analyses, the two mannose derivatives showed superior incorporation efficiencies compared to glucose and galactose isomers. In time-dependent experiments, the incorporation of Ac\(_4\)ManNAz was detectable for up to six days while Ac\(_4\)ManNAl-derived metabolites were absent after two days. Taken together, these findings demonstrate the successful metabolic glycoengineering of immortalized hMSC resulting in transient cell surface modifications, and thus present a useful model to address different scientific questions regarding glycosylation processes in skeletal precursors.
Background
Diagnosis of subscapularis (SSC) tendon lesions on magnetic resonance imaging (MRI) can be challenging. A small coracohumeral distance (CHD) has been associated with SSC tears. This study was designed to define a specific threshold value for CHD to predict SSC tears on axial MRI scans.
Methods
This retrospective study included 172 shoulders of 168 patients who underwent arthroscopic surgery for rotator cuff tear or glenohumeral instability. Diagnostic arthroscopy confirmed an SSC tear in 62 cases (36.0%, test group a), rotator cuff tear tears other than SSC in 71 cases (41.3%, control group b) and glenohumeral instability without any rotator cuff tear in 39 cases (22.7%, zero-sample group c). All patients had a preoperative MRI of the shoulder (1.5T or 3T). Minimum CHD was measured on axial fat-suppressed proton density-, T2-, or T1-weigthed sequences. Receiver operating characteristics analysis was used to determine the threshold value for CHD, and sensitivity and specificity were calculated.
Results
CHD measurement had a good interobserver reliability (Intraclass correlation coefficient 0.799). Mean CHD was highly significantly (P < .001) less for test group a (mean 7.3 mm, standard deviation ± 2.2) compared with control group b (mean 11.1 mm, standard deviation ± 2.3) or zero-sample group c (mean 13.6 mm, standard deviation ± 2.9). A threshold value of CHD <9.5 mm had a sensitivity of 83.6% and a specificity of 83.9% to predict SSC tears.
Conclusion
A CHD <9.5 mm on MRI is predictive of SSC lesions and a valuable tool to diagnose SSC tears.
This review summarizes important information on the ectoenzyme tissue-nonspecific alkaline phosphatase (TNAP) and gives a brief insight into the symptoms, diagnostics, and treatment of the rare disease Hypophosphatasia (HPP), which is resulting from mutations in the TNAP encoding ALPL gene. We emphasize the role of TNAP beyond its well-known contribution to mineralization processes. Therefore, above all, the impact of the enzyme on central molecular processes in the nervous system and on inflammation is presented here.
Locoregional recurrence is a major reason for therapy failure after surgical resection of head and neck squamous cell carcinoma (HNSCC). The physiological process of postoperative wound healing could potentially support the proliferation of remaining tumor cells. The aim of this study was to evaluate the influence of wound fluid (WF) on the cell cycle distribution and a potential induction of epithelial-mesenchymal transition (EMT). To verify this hypothesis, we incubated FaDu and HLaC78 cells with postoperative WF from patients after neck dissection. Cell viability in dependence of WF concentration and cisplatin was measured by flow cytometry. Cell cycle analysis was performed by flow cytometry and EMT-marker expression by rtPCR. WF showed high concentrations of interleukin (IL)-6, IL-8, IL-10, CCL2, MCP-1, EGF, angiogenin, and leptin. The cultivation of tumor cells with WF resulted in a significant increase in cell proliferation without affecting the cell cycle. In addition, there was a significant enhancement of the mesenchymal markers Snail 2 and vimentin, while the expression of the epithelial marker E-cadherin was significantly decreased. After cisplatin treatment, tumor cells incubated with WF showed a significantly higher resistance compared with the control group. The effect of cisplatin-resistance was dependent on the WF concentration. In summary, proinflammatory cytokines are predominantly found in WF. Furthermore, the results suggest that EMT can be induced by WF, which could be a possible mechanism for cisplatin resistance.
Additive manufacturing of scaffolds with sub-micron filaments via melt electrospinning writing

The aim of this study was to explore the lower resolution limits of an electrohydrodynamic process combined with direct writing technology of polymer melts. Termed melt electrospinning writing, filaments are deposited layer-by-layer to produce discrete three-dimensional scaffolds for in vitro research. Through optimization of the parameters (flow rate, spinneret diameter, voltage, collector distance) for poly-ϵ-caprolactone, we could direct-write coherent scaffolds with ultrafine filaments, the smallest being 817 ± 165 nm. These low diameter filaments were deposited to form box-structures with a periodicity of 100.6 ± 5.1 μm and a height of 80 μm (50 stacked filaments; 100 overlap at intersections). We also observed oriented crystalline regions within such ultrafine filaments after annealing at 55 °C. The scaffolds were printed upon NCO-sP(EO-stat-PO)-coated glass slide surfaces and withstood frequent liquid exchanges with negligible scaffold detachment for at least 10 days in vitro.
Purpose: The topical application of tranexamic acid (TXA) into the joint space during total joint arthroplasty (TJA) with no increase of complications, has been widely reported. We investigated the influence of TXA on antibiotic release, activity of the released antibiotic against a clinical isolate of S. aureus, and compressive strength of a widely used commercially prepared gentamicin-loaded cement brand (PALACOS R + G). Method: 12 bone cement cylinders (diameter and height = 6 and 12 mm, respectively) were molded. After curing in air for at least 1 h, six of the cylinders were completely immersed in 5 mL of fetal calf serum (FCS) and the other six were completely immersed in a solution consisting of 4.9 mL of FCS and 0.1 mL (10 mg) of TXA. Gentamicin elution tests were performed over 7 d. Four hundred µL of the gentamicin eluate were taken every 24 h for the first 7 d without renewing the immersion fluid. The gentamicin concentration was determined in a clinical analyzer using a homogeny enzyme immuno-assay. The antimicrobial activity of the eluate, obtained after day 7, was tested. An agar diffusion test regime was used with Staphylococcus aureus. Bacteria were grown in a LB medium and plated on LB agar plates to get a bacterial lawn. Fifty µL of each eluate were pipetted on 12-mm diameter filter discs, which were placed in the middle of the agar gel. After 24 h of cultivation at 37 °C, the zone of inhibition (ZOI) for each specimen was measured. The compressive strength of the cements was determined per ISO 5833. Results: At each time point in the gentamicin release test, the difference in gentamicin concentration, obtained from specimens immersed in the FCS solution only and those immersed in the FCS + TXA solution was not significant (p = 0.055–0.522). The same trend was seen in each of the following parameters, after 7 d of immersion: (1) Cumulative gentamicin concentration (p < 0.297); (2) gentamicin activity against S. aureus (strongly visible); (3) ZOI size (mostly > 20 mm) (p = 0.631); and (4) compressive strength (p = 0.262). Conclusions: For the PALACOS R + G specimens, the addition of TXA to FCS does not produce significant decreases in gentamicin concentration, in the activity of the gentamicin eluate against a clinical isolate of S. aureus, the zone of inhibition of S. aureus, and in the compressive strength of the cement, after 7 d of immersion in the test solution.
In bone tissue engineering, the design of in vitro models able to recreate both the chemical composition, the structural architecture, and the overall mechanical environment of the native tissue is still often neglected. In this study, we apply a bioreactor system where human bone-marrow hMSCs are seeded in human femoral head-derived decellularized bone scaffolds and subjected to dynamic culture, i.e., shear stress induced by continuous cell culture medium perfusion at 1.7 mL/min flow rate and compressive stress by 10% uniaxial load at 1 Hz for 1 h per day. In silico modeling revealed that continuous medium flow generates a mean shear stress of 8.5 mPa sensed by hMSCs seeded on 3D bone scaffolds. Experimentally, both dynamic conditions improved cell repopulation within the scaffold and boosted ECM production compared with static controls. Early response of hMSCs to mechanical stimuli comprises evident cell shape changes and stronger integrin-mediated adhesion to the matrix. Stress-induced Col6 and SPP1 gene expression suggests an early hMSC commitment towards osteogenic lineage independent of Runx2 signaling. This study provides a foundation for exploring the early effects of external mechanical stimuli on hMSC behavior in a biologically meaningful in vitro environment, opening new opportunities to study bone development, remodeling, and pathologies.
Sarcopenia and Malnutrition Screening in Female Osteoporosis Patients — A Cross-Sectional Study

Sarcopenia and malnutrition are important determinants of increased fracture risk in osteoporosis. SARC-F and MNA-SF are well-established questionnaires for identifying patients at risk for these conditions. We sought to evaluate the feasibility and potential added benefit of such assessments as well as the actual prevalence of these conditions in osteoporosis patients. We conducted a cross-sectional, single-center study in female osteoporosis patients ≥ 65 years (SaNSiBaR-study). Results of the sarcopenia (SARC-F) and malnutrition (MNA-SF) screening questionnaires were matched with a functional assessment for sarcopenia and data from patients’ medical records. Out of 107 patients included in the analysis, a risk for sarcopenia (SARC-F ≥ 4 points) and a risk for malnutrition (MNA-SF ≤ 11 points) was found in 33 (30.8%) and 38 (35.5%) patients, respectively. Diagnostic overlap with coincident indicative findings in both questionnaires was observed in 17 patients (16%). As compared to the respective not-at-risk groups, the mean short physical performance battery (SPPB) score was significantly reduced in both patients at risk for sarcopenia (7.0 vs. 10.9 points, p < 0.001) and patients at risk for malnutrition (8.7 vs. 10.5 points, p = 0.005). Still, confirmed sarcopenia according to EWGSOP2 criteria was present in only 6 (6%) of all 107 patients, with only 3 of them having an indicative SARC-F score. Bone mineral density was not significantly different in any of the at-risk groups at any site. In summary, applying SARC-F and MNA-SF in osteoporosis patients appears to be a complementary approach to identify individuals with functional deficits.
Purpose: Patient-specific, flanged acetabular components are used for the treatment of Paprosky type III defects during revision total hip arthroplasty (THA). This monocentric retrospective cohort study analyzes the outcome of patients treated with custom made monoflanged acetabular components (CMACs) with intra- and extramedullary iliac fixation. Methods: 14 patients were included who underwent revision THA with CMACs for the treatment of Paprosky type III defects. Mechanism of THA failure was infection in 4 and aseptic loosening in 10 patients. Seven patients underwent no previous revision, the other seven patients underwent three or more previous revisions. Results: At a mean follow-up of 35.4 months (14–94), the revision rate of the implant was 28.3%. Additionally, one perioperative dislocation and one superficial wound infection occurred. At one year postoperatively, we found a significant improvement of the Western Ontario and McMaster Universities Arthritis Index (WOMAC) score (p = 0.015). Postoperative radiographic analysis revealed good hip joint reconstruction with a mean leg length discrepancy of 3 mm (−8–20), a mean lateralization of the horizontal hip center of rotation of 8 mm (−8–35), and a mean proximalization of the vertical hip center of rotation of 6 mm (13–26). Radiolucency lines were present in 30%. Conclusion: CMACs can be considered an option for the treatment of acetabular bone loss in revision THA. Iliac intra- and extramedullary fixation allows soft tissue-adjusted hip joint reconstruction and improves hip function. However, failure rates are high, with periprosthetic infection being the main threat to successful outcome.
Die Patellaerstluxation ist eine besonders im jüngeren Lebensalter auftretende Verletzungsform. Bei persistierender Instabilität mit Rezidivereignissen ist die Rekonstruktion des medialen patellofemoralen Ligaments (MPFL) ein etabliertes Operationsverfahren.
In dieser Arbeit wurde bei 17 Patienten (Durchschnittsalter 22,65 Jahre) die posturale Stabilität sowie das Kraftverhalten der Oberschenkelmuskulatur im Mittel 400,65 Tage nach Durchführung einer MPFL-Plastik in Form einer klinischen Verlaufsstudie bestimmt.
Die Messung der posturalen Instabilität erfolgte im Einbeinstand auf einem Posturomed (Haider Bioswing). Dabei wurde aus der Ruheposition sowie nach Bewegungsimpuls in AP- und ML-Richtung die Wegstrecke der Standplattform aufgezeichnet. Bei allen Testmodi zeigten sich auf der operierten im Vergleich zur Gegenseite leichtgradig bessere Werte (nicht signifikant).
Die Kraftdiagnostik erfolgte durch isokinetische Testung der Kniegelenksextensoren bzw. -flexoren im Seitenvergleich mittels Biodex System 3 (Medical Systems) zunächst unter konzentrischen Kontraktionsbedingungen bei 60°/s und 240°/s sowie im Anschluss bei exzentrischer Flexion bei 60°/s Winkelgeschwindigkeit. Im ersten Abschnitt zeigten sich auf der betroffenen Seite in die Knieextension niedrigere Werte als auf der Gegenseite (teilweise signifikant) bei keiner wesentlichen Differenz in die Flexion. Im zweiten Teil erzielten die Probanden im Mittel mit ihrem operierten Bein geringere Werte als mit dem nicht operierten Bein (teilweise signifikant).
Zusammenfassend zeigt sich ca. ein Jahr postoperativ kein posturales Defizit jedoch ein Kraftdefizit des Streckapparates der operierten Seite. In der Literatur ist eine postoperative Quadrizepsdsyfunktion nach MPFL-Plastik vielfach beschrieben. Ein möglicher Pathomechanismus ist die arthrogene Muskelinhibition. Die Integration disinhibierender Maßnahmen in herkömmliche Rehabilitationsprogramme stellt einen denkbaren Therapieansatz dar.
Die Frage nach dem idealen Zugangsweg für die komplexe, operative HTEP-Implantation ist seit Jahren Grund zur Diskussion unterschiedlicher Autoren und abschließend noch nicht geklärt. In der Literatur ist in der Vergangenheit über die Vor- und Nachteile der verschiedenen Zugangswege diskutiert, und es sind teilweise auch kritische Stimmen zur minimal-invasiven Operationstechnik laut geworden. Die schlechte Übersichtlichkeit des Operationsgebietes, eine flache Lernkurve, vermehrte postoperative Fehlstellungen und ein schlechtes, funktionelles Outcome werden als Kritikpunkte dieser Technik aufgeführt.
Ein sicherer Zugangsweg, der eine rasche postoperative Mobilisation ermöglicht und zusätzlich den hohen funktionellen und ästhetischen Wünschen der Patienten gerecht wird, liegt im aktuellen Fokus der Forschung in der orthopädischen Endoprothetik und wird durch die Fast-Track Endoprothetik zusätzlich verstärkt. Aus diesem Grund war das Ziel dieser Studie, aussagekräftige Daten des minimal-invasiven anterioren Zugangs, modifiziert nach Smith-Peterson, zum frühen postoperativen Verlauf zu erhalten.
Zur Erhebung der Daten kamen neben krankheitsspezifischen auch krankheitsübergreifende Fragebögen zum Einsatz. Die Ergebnisse der etablierten Fragebögen HHS, XSMFA-D, SF-36, der körperlichen Untersuchung, der Aktivitäts- und Funktionseinschätzung durch den Untersucher sowie durch den Patienten zeigten eine signifikante Besserung bei den Punkten Aktivität, Funktion, Lebensqualität, subjektiven Schmerzwahrnehmung sowie Krankheitsempfinden. Die Aktivität wurde zudem durch den Schrittzähler StepWatch™ (präoperativ) und den TWB (prä- und postoperativ) objektiv gemessen, wobei der TWB im frühen postoperativen Verlauf unter diesen Umständen eine valide Datenerhebung fraglich erscheinen lässt und gegebenfalls einer Revalidierung unterzogen werden muss. Es zeigte sich eine nur geringe Änderung der psychischen Summenskala des PHQ-D zu beiden MZP, obwohl sich die Ergebnisse der Fragebögen zum Thema Funktion, Schmerz und allge-meine Lebensqualität postoperativ signifikant besserten.
Im Vergleich mit den Probanden des zweiten Studienarms, welche eine HTEP durch den transglutealen lateralen Zugang nach Bauer erhielten, zeigten die Daten des funktionellen Outcomes, der subjektiven Aktivität, sowie der Lebensqualität sechs Wochen postoperativ für den minimal-invasiven anterioren Zugang signifikant bessere Ergebnisse. Zusätzlich verringerte sich der Beeinträchtigungsindex postoperativ für die Probanden mit minimal-invasivem anterioren Zugang.
Die Daten dieser Arbeit sind Bestandteil einer Studie, welche im „BMC Musculoskeletal Disorders” im Jahr 2018 veröffentlicht wurde. Wie schon in vorangegangen Studien gezeigt, konnten unsere Daten ein besseres, frühes postoperatives, funktionelles Outcome bei den Patienten mit minimal-invasivem anterioren Zugang untermauern. Obwohl der HHS in beiden Studiengruppen keine signifikante Änderung der Funktion aufzeigen konnte, ergaben sowohl die Daten des Schrittzählers als auch die Ergebnisse des Funktionsindexes des XSMFA-D signifikante Unterschiede in Aktivität und Funktion bei Patienten mit minimal-invasivem anterioren Zugang. Nach zwölf Monaten wiederum zeigte das funktionelle Outcome beider Studienarme keine Differenz mehr. Die Ergebnisse zur gesundheitsbezogenen Lebensqualität ergaben zu allen MZP ein signifikant besseres Ergebnis bei Patienten mit minimal-invasivem anterioren Zugang.
Es konnte schlussendlich gezeigt werden, dass der minimal-invasive anteriore Zugang einen sicheren Weg zum Hüftgelenk darstellt. Vor allem im frühen postoperativen Verlauf, welcher besonders in der „Fast-Track” Endoprothetik relevant ist, zeigte der MIS-Zugang seine Vorzüge. Die Resultate bei den Themen Funktion, Lebensqualität, Schmerzfreiheit, Komplikationsrate und Patientenzufriedenheit lieferten durchweg gute Ergebnisse. Bei der subjektiven Aktivität wurden sogar sehr gute Ergebnisse erzielt.
Schlussfolgernd konnte im Vergleich zum lateralen Zugang nach Bauer gezeigt werden, dass das frühe klinische Outcome mittels minimal-invasivem anterioren Zugang gleichwertige oder sogar bessere Ergebnisse hervorbringt.
Die Arbeit stellt mögliche Einflüsse durch 1,25- Dihydroxy-Vitamin D3 (1,25-VitD3) und Retinsäure (RA) in humanen mesenchymalen Stammzellen (hMSC) sowohl während der adipogenen und osteogenen Differenzierung als auch während der Kurzzeit- und Langzeitstimulation auf das Mikromilieu dar. Die Stimulation mit 1,25-VitD3 und RA verlangsamt das Wachstumsverhalten und verändert die Zellmorphologie von hMSC. Effekte auf die Genexpression werden auf mRNA-Ebene mittels RT-PCR dargestellt. Der Phänotyp als auch teilweise die Genexpression der osteogenen und adipogenen Differenzierung wird durch 1,25-VitD3 induziert und durch RA inhibiert. Zudem wird sowohl die „Mikromilieu-Zusammensetzung“ als auch das „Transkriptionssignal“ von 1,25-VitD3 und RA gegenseitig beeinflusst.
Aufgrund der divergierenden Studienlage bezüglich der physiotherapeutischen Nachbehandlung nach operativer Rotatorenmanschettenrefixation erfolgte im Rahmen einer prospektiv randomisierten Studie die Evaluation zweier Nachbehandlungsmodelle nach operativer Refixation vollschichtiger RM-Rupturen in Mini-Open-Technik. Hierfür wurden 57 Patienten präoperativ, 3 Wochen, 6 Wochen sowie 6 Monate postoperativ nachuntersucht und ausgewertet. Die Scores beinhalteten den NRS-Score, Constant-Score, DASH-Score, ASES-Score, NHP-Score, SF-36-Score sowie eine sonographische Untersuchung zur Beurteilung der Reruptur nach 6 Monaten postoperativ.
Einheitlich erfolgte die Ruhigstellung im Gilchrist-Verband für 6 Wochen. In der konservativen Nachbehandlungsgruppe wurden bis 6 Wochen postoperativ lediglich Pendelübungen durchgeführt, in der progressiven Nachbehandlungsgruppe erfolgte eine passive Beübung direkt postoperativ bis an die Schmerzgrenze mit Ausnahme der Adduktion.
Im Gesamtkollektiv war eine Rerupturrate von 5,3% zu verzeichnen mit 3,7% in der konservativen und 6,7% in der progressiven Nachbehandlungsgruppe ohne signifikanten Gruppenunterschied (p=0,540). Bezüglich der klinischen und psychischen Ergebnisse zeigte sich 6 Monate postoperativ lediglich eine Einschränkung der aktiven Außenrotation in der konservativen Nachbehandlungsgruppe (46,2∘ vs. 39,7∘, p=0,031), sonst war kein signifikanter Gruppenunterschied zu sehen.
Weiterhin erfolgten Subgruppenanalysen insbesondere hinsichtlich Alter und Geschlecht der Patienten. Dabei haben Patienten über 65 Jahren unabhängig von der Nachbehandlungsgruppe kürzer Analgetika eingenommen und waren 6 Wochen postoperativ weniger bewegungseingeschränkt. Aufgrund einer Tendenz zu vermehrten Rerupturen nach progressiver Nachbehandlung in der Literatur werden daher weiterführende Studien benötigt um zu evaluieren, ob ältere Patienten von einer vermehrten Ruhigstellung profitieren könnten.
Diese Studie präsentiert im Gegensatz zu der überwiegend in der Literatur verwendeten arthroskopischen OP-Technik Ergebnisse nach RM-Refixation in Mini-Open-Technik. Damit liefert sie eine gute Grundlage für weiterführende Studien insbesondere in der Behandlung von größeren RM-Rupturen, welche ein erhöhtes Rerupturrisiko besitzen und von einer konservativen Nachbehandlung profitieren könnten.
Die Implantation einer Endoprothese des Kniegelenks ist eine der häufigsten Operationen in Deutschland. In vielen Kliniken wurde im Rahmen des Patient-Blood-Managements ein präoperatives Anämie Screening eingeführt, um mit Hilfe einer präoperativen Anämiebehandlung das Risiko für eine Bluttransfusion und andere Anämie assoziierte Risiken zu senken. In den bisherigen Publikationen variieren jedoch sowohl die Anämieprävalenz als auch das Bluttransfusionsrisiko erheblich, weshalb für eine gezielte Behandlung eine klinikspezifische Analyse von Risikofaktoren für eine Bluttransfusion erforderlich zu sein scheint. Ziel dieser retrospektiven, monozentrischen Studie war es, den Einfluss der präoperativen Anämie und weiterer möglicher Einflussfaktoren auf die Transfusions- und Komplikationsrate nach Implantation einer primären Endoprothese des Kniegelenks zu untersuchen. Hierfür wurden die Daten von 1811 Patienten, welche zwischen 2016 und 2018 eine primäre Endoprothese des Kniegelenks erhalten hatten, erfasst und statistisch ausgewertet. 7,4% der Patienten besaßen eine präoperative Anämie. Patienten mit einer präoperativen Anämie erhielten in 25,4% ein Erythrozytenkonzentrat, nicht anämische Patienten in 3,1% der Fälle. Die gesamte Transfusionsrate für allogene Transfusionen lag bei 4,7%. In der Gruppe der präoperativ anämischen Patienten zeigte sich dabei ein signifikant höhere Wahrscheinlichkeit für die Transfusion eines Erythrozytenkonzentrats, nicht jedoch für eine Komplikation während des stationären Aufenthalts. Unsere logistischen Regressionsmodelle ermittelten neben der präoperativen Hämoglobinkonzentration auch die systemische Tranexamsäuregabe als unabhängigen Einflussfaktor für Transfusionen. Das Geschlecht der Patienten hatte hingegen keinen Einfluss auf das Risiko für eine Bluttransfusion. Männer und Frauen mit der gleichen präoperativen Hämoglobinkonzentration besaßen in etwa das gleiche individuelle Risiko für eine Bluttransfusion. Entsprechend erscheint es sinnvoll, für Männer und Frauen die gleiche präoperative Mindesthämoglobinkonzentration anzustreben. Da jedoch das individuelle Risiko auch durch die systemische Gabe der Tranexamsäure beeinflusst wird, könnte dies bei der Festlegung einer Schwelle zur präoperativen Behandlung berücksichtigt werden.
Hypophosphatasia (HPP) is a rare, inherited, metabolic disease caused by deficient tissue non‐specific alkaline phosphatase activity. This study aims to assess patient‐reported pain, disability and health‐related quality of life (HRQoL) in a real‐world cohort of adults with HPP who were not receiving asfotase alfa during the analysis. Adults (≥18 years old) with HPP (confirmed by ALPL gene mutation and/or low serum alkaline phosphatase activity for age/sex) were identified from the Global HPP Registry (NCT02306720). Demographics, clinical characteristics, and data on patient‐reported pain, disability, and HRQoL (assessed by Brief Pain Inventory Short Form [BPI‐SF], Health Assessment Questionnaire Disability Index [HAQ‐DI], and 36‐Item Short‐Form Health Survey version 2 [SF‐36v2], respectively) were stratified by pediatric‐ and adult‐onset HPP and summarized descriptively. Of the 304 adults included (median [min, max] age 48.6 [18.8, 79.8] years; 74% women), 45% had adult‐onset HPP and 33% had pediatric‐onset HPP (unknown age of onset, 22%). Of those with data, 38% had experienced ≥5 HPP manifestations and 62% had a history of ≥1 fracture/pseudofracture. Median (Q1, Q3) BPI‐SF scores were 3.5 (1.5, 5.3) for pain severity and 3.3 (0.9, 6.2) for pain interference. Median (Q1, Q3) disability on the HAQ‐DI was 0.3 (0.0, 0.7). Median (Q1, Q3) physical and mental component summary scores on the SF‐36v2 were 42.4 (32.7, 49.9) and 45.3 (36.3, 54.8), respectively. Greater numbers of HPP manifestations experienced/body systems affected correlated significantly with poorer scores on the BPI‐SF, HAQ‐DI, and SF‐36v2 (all p < 0.05). No significant differences between adults with pediatric‐ and adult‐onset HPP were observed for patient‐reported outcomes, except for disability and the BPI‐SF question “pain at its worst,” which were significantly higher among adults with pediatric‐ versus adult‐onset HPP (p = 0.03 and 0.04, respectively). These data from the Global HPP Registry show that adults with HPP have a substantial burden of illness that is associated with reduced patient‐reported HRQoL, regardless of age of disease onset.
Patellaluxationen sind eine vor allem bei jungen, aktiven Patienten häufige Verletzung komplexer Ätiologie. Die Rekonstruktion des medialen patellofemoralen Ligaments (MPFL) ist die aktuell etablierte Operationstechnik bei strecknaher patellofemoraler Instabilität, zu der in der Literatur eine Vielzahl an patellaren Fixationstechniken des autologen Sehnentransplantates beschrieben werden.
In dieser Studie wurden 71 Patienten 5 Jahre nach Rekonstruktion des MPFLs mit patellarer Fixation in Weichteiltechnik nachuntersucht und die klinischen Ergebnisse der Operationsmethode und die Zufriedenheit der Patienten ermittelt.
Dafür wurde die Reluxationsrate ermittelt und die Funktion der Kniegelenke im Alltag mithilfe des Kujala- und des Lysholm-Fragebogens, das Aktivitätsniveau der Patienten mit der Tegner-Aktivitätsskala erfasst. Im Rahmen einer Nachuntersuchung wurden die Beweglichkeit des Kniegelenks und die Stabilität der Kniescheibe klinisch untersucht. Die Ergebnisse wurden unter Berücksichtigung klinischer und radiologischer Risikofaktoren ausgewertet.
Die Studie ergab eine Reluxationsrate von 5,6% und ist somit vergleichbar mit der Rate anderer in der Literatur beschriebener Techniken. Die Ergebnisse der klinischen Untersuchung ergaben eine stabile ligamentäre Führung der Kniescheibe bei insgesamt guter Beweglichkeit der Kniegelenke, die Auswertung der Fragebögen zeigten signifikante Verbesserungen der Funktion der operierten Kniegelenke im Alltag bei unverändertem Aktivitätsniveau.
Im Ergebnis kann durch die vorliegende Studie belegt werden, dass durch die Rekonstruktion des MPFL mit weichteiliger patellarer Fixation langfristig gute Ergebnisse bei einer niedrigen Komplikationsrate erzielt werden können. Allerdings erhöht das Zusammentreffen verschiedener Pathologien wie eine Patella alta mit einer ausgeprägten Dysplasie der Trochlea das Risiko für eine persistierende Instabilität und eine erneute Luxation.
Summary
In adult hypophosphatasia (HPP) patients, elevated lumbar spine dual X-ray absorptiometry (DXA) values are associated with markers of disease severity and disease-specific fracture risk while femoral bone mineral density (BMD), being largely unaffected by the disease severity, may still be useful to monitor other causes of increased fracture risk due to low BMD.
Introduction
Hypophosphatasia (HPP) is a rare inherited metabolic disorder due to deficient activity of the tissue-nonspecific alkaline phosphatase (TNAP). Clinical manifestation in adult HPP patients is manifold including an increased risk for fractures, but data regarding clinical significance of DXA measurement and associations with fracture risk and disease severity is scarce.
Methods
Retrospective single-center analysis of DXA scans in patients with confirmed HPP (documented mutation, clinical symptoms, low alkaline phosphatase activity). Further data evaluation included disease-related fractures, laboratory results (alkaline phosphatase, pyridoxalphosphate, phosphoethanolamine), and medical history.
Results
Analysis included 110 patients (84 female, mean age of 46.2 years) of whom 37.3% (n = 41) were harboring two mutations. Average T-Score level at the lumbar spine was − 0.1 (SD 1.9), and mean total hip T-Score was − 1.07 (SD 0.15). Both lower ALP activity and higher substrate levels (pyridoxalphosphate and phosphoethanolamine) were significantly correlated with increased lumbar spine T-Score levels (p < 0.001) while BMD at the hip was not affected by indicators of disease severity. Increased lumbar spine BMD was significantly associated with an increased risk for HPP-related fractures, prevalent in 22 (20%) patients (p < 0.001) with 21 of them having biallelic mutations.
Conclusion
BMD in adult HPP patients is not systematically reduced. Conversely, increased lumbar spine BMD appears to be associated with severely compromised mineralization and increased risk for HPP-related fractures while BMD at the hip appears unaffected by indicators of disease severity, suggesting suitability of this anatomic location for assessing and discerning disorders with increased fracture risk owing to reduced BMD like osteoporosis.

Purpose
This study investigates the redislocation rate and functional outcome at a minimum follow-up of five years after medial patellofemoral ligament (MPFL) reconstruction with soft tissue patellar fixation for patella instability.
Methods
Patients were retrospectively identified and knees were evaluated for trochlea dysplasia according to Dejour, for presence of patella alta and for presence of cartilage lesion at surgery. At a minimum follow-up of five years, information about an incident of redislocation was obtained. Kujala, Lysholm, and Tegner questionnaires as well as range of motion were used to measure functional outcome.
Results
Eighty-nine knees were included. Follow-up rate for redislocation was 79.8% and for functional outcome 58.4%. After a mean follow-up of 5.8 years, the redislocation rate was 5.6%. There was significant improvement of the Kujala score (68.8 to 88.2, p = 0.000) and of the Lysholm score (71.3 to 88.4, p = 0.000). Range of motion at follow-up was 149.0° (115–165). 77.5% of the knees had patella alta and 52.9% trochlear dysplasia types B, C, or D. Patellar cartilage legions were present in 54.2%. Redislocations occurred in knees with trochlear dysplasia type C in combination with patella alta.
Conclusion
MPFL reconstruction with soft tissue patellar fixation leads to significant improvement of knee function and low midterm redislocation rate. Patients with high-grade trochlear dysplasia should be considered for additional osseous correction.
Die Enzyme TNSALP (Tissue Non-Specific Alkaline Phosphatase), ENPP1 (Ectonucleotide Pyrophosphatase/Phosphodiesterase 1) und ANKH (Ankylosis, progressive human homolog) bilden zusammen eine zentrale Regulierungseinheit für den Pyrophosphat (PPi)-Stoffwechsel der Zelle [1, 2].
Störungen dieses genau geregelten Prozesses resultieren in schwerwiegenden Erkrankungen, wie z.B. bei der Hypophosphatasie [3]. Dieser meist autosomal rezessiv vererbten Erkrankung liegt eine durch genetische Mutationen beeinträchtigte Funktion der TNSALP zugrunde, wodurch sich die PPi- Konzentration im Microenvironment der Zelle erhöht. Diese kann im Knochengewebe zu schweren Mineralisierungsstörungen führen [1, 2].
Andere Krankheiten, mit erniedrigten PPi- Konzentrationen, werden mit pathologischen Verkalkungen in verschiedensten Geweben in Verbindung gebracht [4, 5]. Diese gehen unter anderem auf genetische Defekte von ENPP1 zurück[4].
Auch der Mevalonat-Pathway trägt zur Komposition des Microenvironments bezüglich der Homöostase von Phosphaten bei [6, 7]. Hier bestehen auch medizinisch relevante Einflussmöglichkeiten, zum Beispiel durch Bisphosphonate, bei der sogenannten Volkskrankheit Osteoporose.
In dieser Arbeit wurden die Auswirkungen einer PPi-Belastung auf die in vitro Mineralisierung von Mesenchymalen Stammzellen untersucht, wobei Modulatoren der Enzymaktivität für ALP und ENPP1 und der Aktivität des PPi-Kanals ANKH sowie des Mevalonatstoffwechsels zum Einsatz kamen (PPi, Pyridoxalphosphat (PLP), Probenecid, Vitamin D, PPADS (Pyridoxalphosphat-6-azophenyl-2‘,4‘-disulfid Säure) und ß-γmeATP (ß-γ Methylentriphosphat)).
Die Resultate zeigen, dass die Modulation der PPi-Konzentration bei der osteogenen Differenzierung von hMSCs in vitro keine eindeutigen Effekte bewirkt. Geringe Änderungen des Genexpressionsmusters sind letztlich nicht auszuschliessen, blieben jedoch aufgrund der hohen Spendervariabilität durch eine erhöhte Anzahl von Experimenten zu beweisen.
Diese Arbeit zeigt insgesamt eine unerwartet geringe Auswirkung einer exogenen und endogenen Modulation der PPi-Konzentration sowohl mit Blick auf die rein physikalischen Phänomene der Mineralisierung, als auch mit Blick auf die untersuchte Genregulation der wichtigsten beteiligten Proteine, was möglicherweise die hohe Kompensationskapazität der Systeme unter physiologischen Bedingungen reflektiert. Untersuchungen auf proteomischer Ebene, besonders mit Blick auf die Prozessierung von Polypeptiden mit Mineralisierungs-modulierender Wirkung würden möglicherweise genaueren Einblick vermitteln.
Eine genauere Untersuchung der Einflüsse von ENPP1 erscheint für die Zukunft vielversprechend. Allerdings treten hier, besonders auch durch die verwendeten Hemmstoffe der ENPP1, die Phänomene der Vernetzung des Stoffwechsels der Phosphate (inklusive ATP und seiner Metabolite) mit dem Purinergen Signalling deutlich zutage. Diese Vernetzung generiert durch ihre Komplexität sowohl klinisch als auch zellbiologisch/biochemisch erhebliche Interpretationsprobleme, die zukünftige Arbeiten auflösen müssen. Dabei sollte besondere Aufmerksamkeit auf zwei für HPP-PatientInnen klinisch in Zukunft potentiell bedeutsame Ergebnisse gelegt werden, die möglicherweise ungünstigen Auswirkungen einer Therapie mit Probenecid auf die ALPL Expression und die Steigerung der ALPL Expression unter Hemmstoffen des Enzyms ENPP1.

Die vorliegende Studie mit insgesamt 73 Patienten untersucht das klinische und funktionelle Outcome nach Implantation einer kreuzbanderhaltenden patientenspezifischen Kniegelenkstotalendoprothese vom Typ Conformis iTotal® CR G2. Es handelt sich um eine monozentrische retrospektive und deskriptive Studie zu klinischen und radiologischen Ergebnissen zwei, drei sowie fünf Jahre postoperativ. Es wurden zu Vergleichszwecken auch präoperative Daten erhoben und ausgewertet. Neben klinischen und radiologischen Untersuchungen wurden durch die Verwendung des „Knee Society Scores“, des „WOMAC Osteoarthritis Index“ und des „SF-12 Health Survey“-Fragebogens die Ergebnisse bezüglich Kniefunktion, Schmerz und Lebensqualität erhoben. Die Untersuchungen für das mittelfristige Outcome erfolgten im Zeitraum zwischen November 2012 und Januar 2017 unter standardisierten Bedingungen.
Insgesamt zeigte sich im Vergleich zum präoperativen Ausgangswert eine statistisch signifikante Verbesserung aller erhobenen Scores sowie eine verbesserte Funktionalität.
Der Vergleich mit anderen veröffentlichten Studien zeigte eine bessere gesamte Implantationsqualität als bei standardisierten Prothesen. Verglichen mit anderen individualisierten Prothesen sind die Ergebnisse ebenfalls etwas besser bzw. zum Teil gleichwertig. Im Gegensatz zu unserer Studie verbesserten sich die Scores bei den meisten Vergleichsstudien nicht signifikant. Im direkten Vergleich mit den einzelnen Punktzahlen der Scores erzielte die Conformis iTotal® CR G2 Prothese in unserer Studie sehr gute, zum Teil deutlich bessere Ergebnisse.
Trotz der sehr guten und vielversprechenden Ergebnisse sollte aufgrund der deutlich aufwendigeren und strahlenbelastenden präoperativen Maßnahmen, die zur Implantation einer solchen Prothese notwendig sind, sowie der teilweise eingeschränkten Aussagekraft dieser Studie weitere Langzeitstudien bezüglich Funktionalität und Haltbarkeit der Conformis iTotal® CR G2 Prothese durchgeführt werden.
In dieser Arbeit konnte erstmals gezeigt werden, dass plastik-adhärent wachsende, multipotente Vorläuferzellen, die eine für MSCs charakteristische Kombination von Oberflächenantigenen tragen, aus allen vier untersuchten Geweben des arthrotischen Hüftgelenks isoliert werden konnten. MSC-ähnliche Zellen können somit nicht nur in der Spongiosa und im Gelenkknorpel, sondern auch in der anterioren Gelenkkapsel und dem Ligamentum capitis femoris (LCF) des arthrotisch veränderten menschlichen Hüftgelenks nachgewiesen werden.
Die FACS Analyse der Oberflächenantigene auf Zellen, die aus den vier unterschiedlichen Geweben eines beispielhaft gewählten Spenders isoliert wurden, zeigte eine deutliche Expression der Antigene CD44, CD73, CD90 und CD105. Unabhängig vom Nativgewebe zeigten somit alle untersuchten Zellen ein für MSCs charakteristisches, aber nicht spezifisches Profil an Antigenen auf ihrer Oberfläche. Eine Übereinstimmung mit den ISCT Kriterien für MSCs war aufgrund der fehlenden Kontrolle hämatopoetischer Marker nicht möglich.
Die multipotente Differenzierung der isolierten Zellen erfolgte mithilfe spezifischer Differenzierungsmedien in Monolayer-Kulturen oder für die chondrogene Differenzierung in dreidimensionalen Pellet-Kulturen. Nach 21 Tagen konnten in allen differenzierten Kulturen histologisch und immunhistochemisch klare Zeichen der Osteo- und Adipogenese detektiert werden, während die Auswertung spezifischer Markergene eine klare Steigerung der Expression dieser im Vergleich zu den Negativkontrollen zeigte.
Histologische und immunhistochemische Auswertungen bestätigten auch eine erfolgreiche chondrogene Differenzierung der Zell-Pellets aus Spongiosa, Knorpel und Kapsel. Lediglich in den chondrogen differenzierten Zell-Pellets aus dem LCF konnte immunhistochemisch keine Bildung des knorpelspezifischen Matrixproteins Col II nachgewiesen werden. Mikroskopisch zeigten vor allem die differenzierten MSC-Pellets aus Spongiosa und Knorpel morphologisch eine starke Ähnlichkeit zu hyalinem Knorpelgewebe. Trotz dieser Abstufungen zeigten sich für die relative Expression der chondrogenen Markergene AGG, Col II und Sox-9 keine signifikanten Unterschiede zwischen den differenzierten MSC-Kulturen der vier unterschiedlichen Nativgewebe. Ein positiver Nachweis des Markers Col X wies nach 27 Tagen sowohl in differenzierten als auch in undifferenzierten Pellet-Kulturen auf eine leichte chondrogene Hypertrophie hin. Zusammenfassend zeigten sich keine signifikanten Unterschiede im Hinblick auf das osteogene und adipogene Differenzierungspotential aller untersuchten Zellen. Während das chondrogene Differenzierungspotential der Zellen aus Spongiosa, Knorpel und Kapsel sich aus histologischer und immunhistochemischer Sicht ähnelte, zeigten Pellets aus dem LCF ein schwächeres chondrogenes Differenzierungspotential in vitro.
Obwohl somit erstmals MSC-ähnliche Zellen aus dem LCF und Gewebsproben, die neben dem Stratum synoviale auch das Stratum fibrosum der Hüftgelenkskapsel beinhalteten, charakterisiert wurden, sind weitere wissenschaftliche Arbeiten notwendig, um das multipotente Differenzierungspotential dieser Zellen zu optimieren.
Background
The treatment of septic arthritis, caused by either hematogenous seeding, injections, or surgery, can be challenging. Staged reverse shoulder arthroplasty (RSA) with temporary implantation of an antibiotic-loaded spacer is widely accepted but still discussed controversially. This study investigated the shoulder-specific bacterial spectrum, infection control rate, functional outcome, and infection-free survival rate after staged RSA in the mid- to long-term follow-up. It was hypothesized that staged RSA would show a high infection-free survival rate.
Methods
A total of 39 patients treated with staged RSA for primary septic arthritis (n = 8), secondary infection (n = 8), or periprosthetic infection (n = 23) were retrospectively included. The infection control rate was calculated based on cultures taken intraoperatively at spacer removal and RSA implantation. Infection-free survival was defined as no revision due to infection. The minimum follow-up period for functional outcome assessment was 2 years (n = 14; mean, 76 months; range, 31-128 months).
Results
Cutibacterium (26%) and coagulase-negative staphylococci (23%) were the predominant pathogens. The infection control rate was 90%. The cumulative infection-free survival rate was 91% after 128 months. Follow-up examinations showed a mean Constant score of 48 (range, 7-85), a mean QuickDASH (short version of Disabilities of the Arm, Shoulder and Hand questionnaire) score of 40.0 (range, 11.4-93.3), and a mean pain score of 1.6 (range, 0-7).
Conclusion
Staged RSA implantation was confirmed to be a reliable treatment option for primary, secondary, and periprosthetic infections of the shoulder. The infection control rate and infection-free survival rate are satisfactory. However, patients and surgeons must be aware of functional impairment even after successful treatment of infections.
Background
For improved outcomes in total knee arthroplasty (TKA) correct implant fitting and positioning are crucial. In order to facilitate a best possible implant fitting and positioning patient-specific systems have been developed. However, whether or not these systems allow for better implant fitting and positioning has yet to be elucidated. For this reason, the aim was to analyse the novel patient-specific cruciate retaining knee replacement system iTotal (TM) CR G2 that utilizes custom-made implants and instruments for its ability to facilitate accurate implant fitting and positioning including correction of the hip-knee-ankle angle (HKA).
Methods
We assessed radiographic results of 106 patients who were treated with the second generation of a patient-specific cruciate retaining knee arthroplasty using iTotal\(^{TM}\) CR G2 (ConforMIS Inc.) for tricompartmental knee osteoarthritis (OA) using custom-made implants and instruments. The implant fit and positioning as well as the correction of the mechanical axis (hip-knee-ankle angle, HKA) and restoration of the joint line were determined using pre- and postoperative radiographic analyses.
Results
On average, HKA was corrected from 174.4 degrees +/- 4.6 degrees preoperatively to 178.8 degrees +/- 2.2 degrees postoperatively and the coronal femoro-tibial angle was adjusted on average 4.4 degrees. The measured preoperative tibial slope was 5.3 degrees +/- 2.2 degrees (mean +/- SD) and the average postoperative tibial slope was 4.7 degrees +/- 1.1 degrees on lateral views. The joint line was well preserved with an average modified Insall-Salvati index of 1.66 +/- 0.16 pre- and 1.67 +/- 0.16 postoperatively. The overall accuracy of fit of implant components was decent with a measured medial overhang of more than 1 mm (1.33 mm +/- 0.32 mm) in 4 cases only. Further, a lateral overhang of more than 1 mm (1.8 mm +/- 0.63) (measured in the anterior-posterior radiographs) was observed in 11 cases, with none of the 106 patients showing femoral notching.
Conclusion
The patient-specific iTotal\(^{TM}\) CR G2 total knee replacement system facilitated a proper fitting and positioning of the implant components. Moreover, a good restoration of the leg axis towards neutral alignment was achieved as planned. Nonetheless, further clinical follow-up studies are necessary to validate our findings and to determine the long-term impact of using this patient- specific system.
Dynamic resistance exercise (DRT) might be the most promising agent for fighting sarcopenia in older people. However, the positive effect of DRT on osteopenia/osteoporosis in men has still to be confirmed. To evaluate the effect of low‐volume/high‐intensity (HIT)‐DRT on bone mineral density (BMD) and skeletal muscle mass index (SMI) in men with osteosarcopenia, we initiated the Franconian Osteopenia and Sarcopenia Trial (FrOST). Forty‐three sedentary community‐dwelling older men (aged 73 to 91 years) with osteopenia/osteoporosis and SMI‐based sarcopenia were randomly assigned to a HIT‐RT exercise group (EG; n = 21) or a control group (CG; n = 22). HIT‐RT provided a progressive, periodized single‐set DRT on machines with high intensity, effort, and velocity twice a week, while CG maintained their lifestyle. Both groups were adequately supplemented with whey protein, vitamin D, and calcium. Primary study endpoint was integral lumbar spine (LS) BMD as determined by quantitative computed tomography. Core secondary study endpoint was SMI as determined by dual‐energy X‐ray absorptiometry. Additional study endpoints were BMD at the total hip and maximum isokinetic hip−/leg‐extensor strength (leg press). After 12 months of exercise, LS‐BMD was maintained in the EG and decreased significantly in the CG, resulting in significant between‐group differences (p < 0.001; standardized mean difference [SMD] = 0.90). In parallel, SMI increased significantly in the EG and decreased significantly in the CG (p < 0.001; SMD = 1.95). Total hip BMD changes did not differ significantly between the groups (p = 0.064; SMD = 0.65), whereas changes in maximum hip−/leg‐extensor strength were much more prominent (p < 0.001; SMD = 1.92) in the EG. Considering dropout (n = 2), attendance rate (95%), and unintended side effects/injuries (n = 0), we believe our HIT‐RT protocol to be feasible, attractive, and safe. In summary, we conclude that our combined low‐threshold HIT‐RT/protein/vitamin D/calcium intervention was feasible, safe, and effective for tackling sarcopenia and osteopenia/osteoporosis in older men with osteosarcopenia.
Background
Hypophosphatasia (HPP) is a rare, inherited metabolic disorder caused by loss-of-function mutations in the ALPL gene that encodes the tissue-nonspecific alkaline phosphatase TNAP (ORPHA 436). Its clinical presentation is highly heterogeneous with a remarkably wide-ranging severity. HPP affects patients of all ages. In children HPP-related musculoskeletal symptoms may mimic rheumatologic conditions and diagnosis is often difficult and delayed. To improve the understanding of HPP in children and in order to shorten the diagnostic time span in the future we studied the natural history of the disease in our large cohort of pediatric patients. This single centre retrospective chart review included longitudinal data from 50 patients with HPP diagnosed and followed at the University Children's Hospital Wuerzburg, Germany over the last 25 years.
Results
The cohort comprises 4 (8%) perinatal, 17 (34%) infantile and 29 (58%) childhood onset HPP patients. Two patients were deceased at the time of data collection. Diagnosis was based on available characteristic clinical symptoms (in 88%), low alkaline phosphatase (AP) activity (in 96%), accumulating substrates of AP (in 58%) and X-ray findings (in 48%). Genetic analysis was performed in 48 patients (31 compound heterozygous, 15 heterozygous, 2 homozygous mutations per patient), allowing investigations on genotype-phenotype correlations. Based on anamnestic data, median age at first clinical symptoms was 3.5 months (min. 0, max. 107), while median time to diagnosis was 13 months (min. 0, max. 103). Common symptoms included: impairment of motor skills (78%), impairment of mineralization (72%), premature loss of teeth (64%), musculoskeletal pain and craniosynostosis (each 64%) and failure to thrive (62%). Up to now 20 patients started medical treatment with Asfotase alfa.
Conclusions
Reported findings support the clinical perception of HPP being a chronic multi-systemic disease with often delayed diagnosis. Our natural history information provides detailed insights into the prevalence of different symptoms, which can help to improve and shorten diagnostics and thereby lead to an optimised medical care, especially with promising therapeutic options such as enzyme-replacement-therapy with Asfotase alfa in mind.
Objective
As native cartilage consists of different phenotypical zones, this study aims to fabricate different types of neocartilage constructs from collagen hydrogels and human mesenchymal stromal cells (MSCs) genetically modified to express different chondrogenic factors.
Design
Human MSCs derived from bone-marrow of osteoarthritis (OA) hips were genetically modified using adenoviral vectors encoding sex-determining region Y-type high-mobility-group-box (SOX)9,transforming growth factor beta (TGFB) 1or bone morphogenetic protein (BMP) 2cDNA, placed in type I collagen hydrogels and maintained in serum-free chondrogenic media for three weeks. Control constructs contained unmodified MSCs or MSCs expressing GFP. The respective constructs were analyzed histologically, immunohistochemically, biochemically, and by qRT-PCR for chondrogenesis and hypertrophy.
Results
Chondrogenesis in MSCs was consistently and strongly induced in collagen I hydrogels by the transgenesSOX9,TGFB1andBMP2as evidenced by positive staining for proteoglycans, chondroitin-4-sulfate (CS4) and collagen (COL) type II, increased levels of glycosaminoglycan (GAG) synthesis, and expression of mRNAs associated with chondrogenesis. The control groups were entirely non-chondrogenic. The levels of hypertrophy, as judged by expression of alkaline phosphatase (ALP) and COL X on both the protein and mRNA levels revealed different stages of hypertrophy within the chondrogenic groups (BMP2>TGFB1>SOX9).
Conclusions
Different types of neocartilage with varying levels of hypertrophy could be generated from human MSCs in collagen hydrogels by transfer of genes encoding the chondrogenic factorsSOX9,TGFB1andBMP2. This technology may be harnessed for regeneration of specific zones of native cartilage upon damage.
Background
The role of cement-augmented screw fixation for calcaneal fracture treatment remains unclear. Therefore, this study was performed to biomechanically analyze screw osteosynthesis by reinforcement with either a calcium phosphate (CP)-based or polymethylmethacrylate (PMMA)-based injectable bone cement.
Methods
A calcaneal fracture (Sanders type IIA) including a central cancellous bone defect was generated in 27 synthetic bones, and the specimens were assigned to 3 groups. The first group was fixed with four screws (3.5 mm and 6.5 mm), the second group with screws and CP-based cement (Graftys (R) QuickSet; Graftys, Aix-en-Provence, France), and the third group with screws and PMMA-based cement (Traumacem (TM) V+; DePuy Synthes, Warsaw, IN, USA). Biomechanical testing was conducted to analyze peak-to-peak displacement, total displacement, and stiffness in following a standardized protocol.
Results
The peak-to-peak displacement under a 200-N load was not significantly different among the groups; however, peak-to-peak displacement under a 600- and 1000-N load as well as total displacement exhibited better stability in PMMA-augmented screw osteosynthesis compared to screw fixation without augmentation. The stiffness of the construct was increased by both CP- and PMMA-based cements.
Conclusion
Addition of an injectable bone cement to screw osteosynthesis is able to increase fixation strength in a biomechanical calcaneal fracture model with synthetic bones. In such cases, PMMA-based cements are more effective than CP-based cements because of their inherently higher compressive strength. However, whether this high strength is required in the clinical setting for early weight-bearing remains controversial, and the non-degradable properties of PMMA might cause difficulties during subsequent interventions in younger patients.
Vitamin D deficiency is a global health concern that is estimated to afflict over one billion people globally. The major role of vitamin D is that of a regulator of calcium and phosphate metabolism, thus, being essential for proper bone mineralisation. Concomitantly, vitamin D is known to exert numerous extra-skeletal actions. For example, it has become evident that vitamin D has direct anti-proliferative, pro-differentiation and pro-apoptotic actions on cancer cells. Hence, vitamin D deficiency has been associated with increased cancer risk and worse prognosis in several malignancies. We have recently demonstrated that vitamin D deficiency promotes secondary cancer growth in bone. These findings were partly attributable to an increase in bone remodelling but also through direct effects of vitamin D on cancer cells. To date, very little is known about vitamin D status of patients with bone tumours in general. Thus, the objective of this study was to assess vitamin D status of patients with diverse bone tumours. Moreover, the aim was to elucidate whether or not there is an association between pre-diagnostic vitamin D status and tumour malignancy in patients with bone tumours.
In a multi-center analysis, 25(OH)D, PTH and calcium levels of 225 patients that presented with various bone tumours between 2017 and 2018 were assessed. Collectively, 76% of all patients had insufficient vitamin D levels with a total mean 25(OH)D level of 21.43 ng/ml (53.58 nmol/L). In particular, 52% (117/225) of patients were identified as vitamin D deficient and further 24% of patients (55/225) were vitamin D insufficient. Notably, patients diagnosed with malignant bone tumours had significantly lower 25(OH)D levels than patients diagnosed with benign bone tumours [19.3 vs. 22.75 ng/ml (48.25 vs. 56.86 nmol/L); p = 0.04).
In conclusion, we found a widespread and distressing rate of vitamin D deficiency and insufficiency in patients with bone tumours. However, especially for patients with bone tumours sufficient vitamin D levels seem to be of great importance. Thus, we believe that 25(OH)D status should routinely be monitored in these patients. Collectively, there should be an increased awareness for physicians to assess and if necessary correct vitamin D status of patients with bone tumours in general or of those at great risk of developing bone tumours.
Hypophosphatasia (HPP) is a rare genetic disease with diverse symptoms and a heterogeneous severity of onset with underlying mutations in the ALPL gene encoding the ectoenzyme Tissue-nonspecific alkaline phosphatase (TNAP). Considering the establishment of zebrafish (Danio rerio) as a new model organism for HPP, the aim of the study was the spatial and temporal analysis of alpl expression in embryos and adult brains. Additionally, we determined functional consequences of Tnap inhibition on neural and skeletal development in zebrafish. We show that expression of alpl is present during embryonic stages and in adult neuronal tissues. Analyses of enzyme function reveal zones of pronounced Tnap-activity within the telencephalon and the mesencephalon. Treatment of zebrafish embryos with chemical Tnap inhibitors followed by axonal and cartilage/mineralized tissue staining imply functional consequences of Tnap deficiency on neuronal and skeletal development. Based on the results from neuronal and skeletal tissue analyses, which demonstrate an evolutionary conserved role of this enzyme, we consider zebrafish as a promising species for modeling HPP in order to discover new potential therapy strategies in the long-term.
Background
While multiple in vitro studies examined mesenchymal stromal cells (MSCs) derived from bone marrow or hyaline cartilage, there is little to no data about the presence of MSCs in the joint capsule or the ligamentum capitis femoris (LCF) of the hip joint. Therefore, this in vitro study examined the presence and differentiation potential of MSCs isolated from the bone marrow, arthritic hyaline cartilage, the LCF and full-thickness samples of the anterior joint capsule of the hip joint.
Methods
MSCs were isolated and multiplied in adherent monolayer cell cultures. Osteogenesis and adipogenesis were induced in monolayer cell cultures for 21 days using a differentiation medium containing specific growth factors, while chondrogenesis in the presence of TGF-ss1 was performed using pellet-culture for 27 days. Control cultures were maintained for comparison over the same duration of time. The differentiation process was analyzed using histological and immunohistochemical stainings as well as semiquantitative RT-PCR for measuring the mean expression levels of tissue-specific genes.
Results
This in vitro research showed that the isolated cells from all four donor tissues grew plastic-adherent and showed similar adipogenic and osteogenic differentiation capacity as proven by the histological detection of lipid droplets or deposits of extracellular calcium and collagen type I. After 27 days of chondrogenesis proteoglycans accumulated in the differentiated MSC-pellets from all donor tissues. Immunohistochemical staining revealed vast amounts of collagen type II in all differentiated MSC-pellets, except for those from the LCF. Interestingly, all differentiated MSCs still showed a clear increase in mean expression of adipogenic, osteogenic and chondrogenic marker genes. In addition, the examination of an exemplary selected donor sample revealed that cells from all four donor tissues were clearly positive for the surface markers CD44, CD73, CD90 and CD105 by flow cytometric analysis.
Conclusions
This study proved the presence of MSC-like cells in all four examined donor tissues of the hip joint. No significant differences were observed during osteogenic or adipogenic differentiation depending on the source of MSCs used. Further research is necessary to fully determine the tripotent differentiation potential of cells isolated from the LCF and capsule tissue of the hip joint.
Background: The soft tissue of the central pretibial area is difficult to reconstruct often requiring free tissue transfer. Especially medi- cally compromised patients are not ideal candidates for free tissue transfer and may benefit from expeditiously harvested local flaps with limited donor site morbidity. As muscle flaps are rare, pedi- cled flaps based on lateral perforators represent an alternative as the arc of rotation can often be limited to 90 °.
Material and Methods: A retrospective analysis of patient data was conducted to identify patients over the age of 60 years with comor- bidities that underwent pretibial soft tissue reconstruction with a single-pedicle perforator flap. Patient demographics, size and cause of the defect, flap dimension, arc of rotation and complications were recorded.
Results: Five patients with an average age of 71.4 years were in- cluded. The arc of rotation was 69 °, all flaps healed. There were two recurrences of osteomyelitis.
Conclusion: Lateral perforators originating from the anterior tib- ial artery or peroneal artery are adequate source vessels for single pedicled perforator flaps even in medically compromised patients. A perforator located proximal to the defect allows limiting the arcof rotation to less than 90 °, which increases the safety of the flap. Patients benefit from a simple procedure without a microvascular anastomosis and a donor site confined to one extremity
Background
Surgical reattachment of the tendon is still the gold standard for ruptures of the distal biceps brachii tendon. Several fixation techniques have been described in the literature, with suture anchors being one of the most common fixation techniques. Currently, there is no data available on how many anchors are required for a safe and stable refixation. In this case report clinical data of a patient with non-simultaneous bilateral distal biceps tendon ruptures treated with a different number of suture anchors for each side (one vs. two) are demonstrated.
Case presentation
A 47-year-old factory worker suffered a rupture of the distal biceps tendon on both arms following two different occasions. The left side was fixed using a single suture anchor, while refixation on the right side was performed with two anchors. The patient was prospectively followed for one year. Functional outcome was assessed using the Andrews Carson Score (ACS), the Oxford Elbow Score (OES), and the Disabilities of Arm, Shoulder and Hand (DASH) Score after six, twelve, 24 and 48 weeks. Furthermore, an isokinetic strength measurement for flexion strength was performed after 24 and 48 weeks. After 48 weeks the patient presented with excellent functional outcome scores and no follow-up complications. During the follow-up period, no differences in the functional scores nor in the isokinetic flexion strength measurement could be detected. Furthermore, no radiological complications (like heterotopic ossifications) could be detected in the postoperative radiographs after one year.
Conclusions
Anatomic reattachment of the distal biceps tendon is a successful operative treatment option for distal biceps tendon ruptures. Suture anchor fixation remains one of the most common techniques, as it allows fast surgery and provides good results with respect to range of motion (ROM) and functional scoring according to the current literature. However, the number of anchors required for a stable fixation remains unclear. As indicated by our presented case, we hypothesize, that there are no significant differences between a one-point or a two-point fixation. In the presented case report, no intraindividual differences between the usage of one versus two suture anchors were evident in the short-term follow-up.
Background
Mesenchymal stem cell (MSC) based-treatments of cartilage injury are promising but impaired by high levels of hypertrophy after chondrogenic induction with several bone morphogenetic protein superfamily members (BMPs). As an alternative, this study investigates the chondrogenic induction of MSCs via adenoviral gene-delivery of the transcription factor SOX9 alone or in combination with other inducers, and comparatively explores the levels of hypertrophy and end stage differentiation in a pellet culture system in vitro.
Methods
First generation adenoviral vectors encoding SOX9, TGFB1 or IGF1 were used alone or in combination to transduce human bone marrow-derived MSCs at 5 x 10\(^2\) infectious particles/cell. Thereafter cells were placed in aggregates and maintained for three weeks in chondrogenic medium. Transgene expression was determined at the protein level (ELISA/Western blot), and aggregates were analysed histologically, immunohistochemically, biochemically and by RT-PCR for chondrogenesis and hypertrophy.
Results
SOX9 cDNA was superior to that encoding TGFB1, the typical gold standard, as an inducer of chondrogenesis in primary MSCs as evidenced by improved lacuna formation, proteoglycan and collagen type II staining, increased levels of GAG synthesis, and expression of mRNAs associated with chondrogenesis. Moreover, SOX9 modified aggregates showed a markedly lower tendency to progress towards hypertrophy, as judged by expression of the hypertrophy markers alkaline phosphatase, and collagen type X at the mRNA and protein levels.
Conclusion
Adenoviral SOX9 gene transfer induces chondrogenic differentiation of human primary MSCs in pellet culture more effectively than TGFB1 gene transfer with lower levels of chondrocyte hypertrophy after 3 weeks of in vitro culture. Such technology might enable the formation of more stable hyaline cartilage repair tissues in vivo.
Objective
This study aimed to compare a state‐of‐the‐art bioelectrical impedance analysis (BIA) device with two‐point Dixon magnetic resonance imaging (MRI) for the quantification of visceral adipose tissue (VAT) as a health‐related risk factor.
Methods
A total of 63 male participants were measured using a 3‐T MRI scanner and a segmental, multifrequency BIA device. MRI generated fat fraction (FF) maps, in which VAT volume, total abdominal adipose tissue volume, and FF of visceral and total abdominal compartments were quantified. BIA estimated body fat mass and VAT area.
Results
Coefficients of determination between abdominal (r\(^{2}\) = 0.75) and visceral compartments (r\(^{2}\) = 0.78) were similar for both groups, but slopes differed by a factor of two. The ratio of visceral to total abdominal FF was increased in older men compared with younger men. This difference was not detected with BIA. MRI and BIA measurements of the total abdominal volume correlated moderately (r\(^{2}\) = 0.31‐0.56), and visceral measurements correlated poorly (r\(^{2}\) = 0.13‐0.44).
Conclusions
Visceral BIA measurements agreed better with MRI measurements of the total abdomen than of the visceral compartment, indicating that BIA visceral fat area assessment cannot differentiate adipose tissue between visceral and abdominal compartments in young and older participants.
The topical application of tranexamic acid (TXA) helps to prevent post-operative blood loss in total joint replacements. Despite these findings, the effects on articular and periarticular tissues remain unclear. Therefore, this in vitro study examined the effects of varying exposure times and concentrations of TXA on proliferation rates, gene expression and differentiation capacity of chondrocytes and human mesenchymal stromal cells (hMSCs), which underwent osteogenic differentiation. Chondrocytes and hMSCs were isolated and multiplied in monolayer cell cultures. Osteogenic differentiation of hMSCs was induced for 21 days using a differentiation medium containing specific growth factors. Cell proliferation was analyzed using ATP assays. Effects of TXA on cell morphology were examined via light microscopy and histological staining, while expression levels of tissue-specific genes were measured using semiquantitative RT-PCR. After treatment with 50 mg/mL of TXA, a decrease in cell proliferation rates was observed. Furthermore, treatment with concentrations of 20 mg/mL of TXA for at least 48 h led to a visible detachment of chondrocytes. TXA treatment with 50 mg/mL for at least 24 h led to a decrease in the expression of specific marker genes in chondrocytes and osteogenically differentiated hMSCs. No significant effects were observed for concentrations beyond 20 mg/mL of TXA combined with exposure times of less than 24 h. This might therefore represent a safe limit for topical application in vivo. Further research regarding in vivo conditions and effects on hMSC functionality are necessary to fully determine the effects of TXA on articular and periarticular tissues.
Tissue-nonspecific alkaline phosphatase (TNAP) is a ubiquitously expressed enzyme that is best known for its role during mineralization processes in bones and skeleton. The enzyme metabolizes phosphate compounds like inorganic pyrophosphate and pyridoxal-5′-phosphate to provide, among others, inorganic phosphate for the mineralization and transportable vitamin B6 molecules. Patients with inherited loss of function mutations in the ALPL gene and consequently altered TNAP activity are suffering from the rare metabolic disease hypophosphatasia (HPP). This systemic disease is mainly characterized by impaired bone and dental mineralization but may also be accompanied by neurological symptoms, like anxiety disorders, seizures, and depression. HPP characteristically affects all ages and shows a wide range of clinical symptoms and disease severity, which results in the classification into different clinical subtypes. This review describes the molecular function of TNAP during the mineralization of bones and teeth, further discusses the current knowledge on the enzyme’s role in the nervous system and in sensory perception. An additional focus is set on the molecular role of TNAP in health and on functional observations reported in common laboratory vertebrate disease models, like rodents and zebrafish.
The incidence of musculoskeletal diseases is steadily increasing with aging of the population. In the past years, extracellular vesicles (EVs) have gained attention in musculoskeletal research. EVs have been associated with various musculoskeletal pathologies as well as suggested as treatment option. EVs play a pivotal role in communication between cells and their environment. Thereby, the EV cargo is highly dependent on their cellular origin. In this review, we summarize putative mechanisms by which EVs can contribute to musculoskeletal tissue homeostasis, regeneration and disease, in particular matrix remodeling and mineralization, pro-angiogenic effects and immunomodulatory activities. Mesenchymal stromal cells (MSCs) present the most frequently used cell source for EV generation for musculoskeletal applications, and herein we discuss how the MSC phenotype can influence the cargo and thus the regenerative potential of EVs. Induced pluripotent stem cell-derived mesenchymal progenitor cells (iMPs) may overcome current limitations of MSCs, and iMP-derived EVs are discussed as an alternative strategy. In the last part of the article, we focus on therapeutic applications of EVs and discuss both practical considerations for EV production and the current state of EV-based therapies.
Hypophosphatasia (HPP) is a rare, inherited, metabolic disease characterized by tissue‐nonspecific alkaline phosphatase deficiency resulting in musculoskeletal and systemic clinical manifestations. This observational study evaluated the effectiveness of enzyme replacement therapy with asfotase alfa on physical function and health‐related quality of life (HRQoL) among adults with pediatric‐onset HPP who received asfotase alfa for 12 months at a single center (ClinicalTrial.gov no.: NCT03418389). Primary outcomes evaluated physical function with the 6‐minute walk test (6MWT), timed up‐and‐go (TUG) test, Short Physical Performance Battery (SPPB), and handheld dynamometry (HHD). Secondary outcome measures included the Lower Extremity Functional Scale (LEFS), pain prevalence/intensity, and pain medication use; HRQoL was evaluated using the 36‐Item Short‐Form Health Survey version 2 (SF‐36v2). Safety data were collected throughout the study. All 14 patients (11 women) had compound heterozygous ALPL gene mutations and ≥1 HPP bone manifestation, including history of ≥1 fracture. Mean (min, max) age was 51 (19 to 78) years. From baseline to 12 months of treatment, median 6MWT distance increased from 267 m to 320 m (n = 13; p = 0.023); median TUG test time improved from 14.4 s to 11.3 s (n = 9; p = 0.008). Specific components of the SPPB also improved significantly: median 4‐m gait speed increased from 0.8 m/s to 1.1 m/s (n = 10; p = 0.007) and median repeated chair‐rise time improved from 22 s to 13 s (n = 9; p = 0.008). LEFS score improved from 24 points to 53 points (n = 10; p = 0.002). Improvements in HHD were not clinically significant. SF‐36v2 Physical Component Score (PCS) improved after 12 months of treatment (n = 9; p = 0.010). Pain level did not change significantly from baseline to 12 months of treatment. There were significant improvements on chair‐rise time and SF‐36v2 PCS by 3 months, and on TUG test time after 6 months. No new safety signals were identified. These results show the real‐world effectiveness of asfotase alfa in improving physical functioning and HRQoL in adults with pediatric‐onset HPP. © 2020 The Authors. Die Revisionsendoprothetik des Hüftgelenkes ist eine große Herausforderung, insbesondere bei Teilwechseln von Komponenten bei fest-implantiertem Schaft. In einer retrospektiven Analyse wurde eine Implantat-Komponente des Medizinprodukteherstellers Merete Medical (Berlin) untersucht. Ziel der Untersuchung war es, eine Aussage über die Stabilität des Implantats und den klinischen Versorgungserfolg bei dessen Verwendung treffen zu können. Es wurden Daten von insgesamt 72 Patienten erhoben. Es erfolgte eine klinische und radiologische Nachuntersuchung. Die Daten wurden einer deskriptiven und explorativen Analyse unterzogen, eine Überlebenszeitanalyse wurde durchgeführt. Das Implantat zeigte mechanische Stabilität in einem mittelfristigen Zeitraum. Der Versorgungserfolg (gemessen am Harris Hip Score) war durchschnittlich bis gut. Es zeigten sich Zusammenhänge zwischen dem präoperativen Zustand der Patienten und dem Versorgungerfolg.
Revision Arthroplasty Through the Direct Anterior Approach Using an Asymmetric Acetabular Component

Despite increasing numbers of primary hip arthroplasties performed through the direct anterior approach (DAA), there is a lack of literature on DAA revision arthroplasty. The present study was performed in order to evaluate outcomes and revision rates after revision through the DAA using an asymmetric acetabular component with optional intra- and extramedullary fixation. In a retrospective cohort study, we analyzed prospectively collected data of 57 patients (61 hips, 43 female, 18 male) who underwent aseptic acetabular component revision through the DAA with the abovementioned implant system between January 2015 and December 2017. The mean follow-up was 40 months (12–56). Survival rates were estimated using the Kaplan–Meier method. All complications were documented and functional outcomes were assessed pre- and postoperatively. Kaplan–Meier analysis revealed an estimated five-year implant survival of 97% (confidence interval CI 87–99%). The estimated five-year survival with revision for any cause was 93% (CI 83–98%). The overall revision rate was 6.6% (n = 4). Two patients had to undergo revision due to periprosthetic infection (3.3%). In one patient, the acetabular component was revised due to aseptic loosening four months postoperatively. Another patient suffered from postoperative iliopsoas impingement and was treated successfully by arthroscopic iliopsoas tenotomy. Two (3.3%) of the revised hips dislocated postoperatively. The mean Harris Hip Score improved from 35 (2–66) preoperatively to 86 (38–100) postoperatively (p < 0.001). The hip joint’s anatomical center of rotation was restored at a high degree of accuracy. Our findings demonstrate that acetabular revision arthroplasty through the DAA using an asymmetric acetabular component with optional intra- and extramedullary fixation is safe and practicable, resulting in good radiographic and clinical midterm results.
Hintergrund: Die Entwicklung minimal-invasiver gewebeschonender Zugangswege in der Hüftendoprothetik ist im Allgemeinen mit einer schnelleren Rehabilitation, einer Verringerung der postoperativen Schmerzen und einem erhöhten Patientenkomfort verbunden. Das Ziel dieser Studie war es, einen anterioren minimal-invasiven mit einem transglutealen lateralen Zugangsweg für Hüftgelenksersatzoperationen in Bezug auf postoperatives Schmerzniveau, den Schmerzmittelgebrauch, die Länge des Krankenhausaufenthalts und die Zeit bis zum Erreichen eines definierten physiotherapeutischen Therapiezieles zu vergleichen.
Methoden: In dieser retrospektiven Kohortenstudie untersuchten wir 200 Patienten, die sich einer Hüftendoprothesenimplantation unterzogen. Gruppe I (n = 100) erhielt einen minimal-invasiven anterioren Hüftzugang (modifiziert nach Smith-Peterson), Gruppe II (n = 100) erhielt einen lateralen transglutealen Zugang (nach Bauer). Die Untersuchungsziele waren der Schmerz in Ruhe und während der Physiotherapie, der Schmerzmittelgebrauch, die Länge des stationären Aufenthaltes sowie die Zeitdauer bis zum Erreichen des physiotherapeutischen Therapiezieles.
Ergebnisse: Die Patienten der Gruppe I konsumierten weniger Schmerzmedikamente (19,6 ± 6,9 mg Piritramid gegenüber 23,6 ± 11,3 mg; p = 0,005) und hatten in der frühen postoperativen Phase weniger Schmerzen (Operationstag: VAS 1,3 ± 1 gegenüber 2,3 ± 1,3, p = 0,0001, erster postoperativer Tag: VAS 0,41 ± 0,8 vs. 0,66 ± 1,1, p = 0,036). Die Zeit bis zum Erreichen des definierten Therapiezieles (6,4 ± 2 Tage gegenüber 7,4 ± 2,1 Tagen; p = 0,001) und die Dauer des Krankenhausaufenthalts waren kürzer (10,2 ± 1,9 Tage gegenüber 13,4 ± 1,6 Tagen; p = 0,0001) für Gruppe I. Die Schmerzen während der Physiotherapie waren jedoch am dritten und sechsten bis neunten Tag nach der Operation im Vergleich zu Gruppe II höher (p = 0,001–0,013).
Schlussfolgerung: Die Implantation einer Hüftprothese über einen minimal-invasiven anterioren Zugang führt in der direkt postoperativen Phase zu einer Reduktion des Schmerzniveaus und des Schmerzmittelgebrauchs. Zusätzlich werden das Erreichen des physiotherapeutischen Therapiezieles und die Länge des stationären Aufenthaltes positiv beeinflusst. Den höheren Schmerzen während der Physiotherapie könnte durch eine Reduktion der erlaubten Gewichtsbelastung in der frühen postoperativen Phase entgegengewirkt werden.
Osteoporose wird definiert als erworbene, generalisierte Skeletterkrankung, die durch eine verminderte Knochenfestigkeit und einen pathologischen Knochenverlust charakterisiert wird. Durch die Störung der Mikroarchitektur kommt es zu strukturellen und funktionellen Defiziten im Sinne von Fragilitätsfrakturen. Mechanische Stimulation erhält die Gewebemasse und stimuliert deren kontinuierliche Anpassung. Östrogene spielen bei der Entwicklung, dem Wachstum und der Regeneration des Knochens eine bedeutende Rolle und wirken über Bindung an die Östrogenrezeptoren ER und ER in bestimmten Zielgeweben. Östrogenrezeptoren sind unverändert sehr geeignete Targets für die Entwicklung von Medikamenten im Rahmen der Osteoporosetherapie wie z.B. die selektiven Östrogen-Rezeptor-Modulatoren (SERMs). Die molekulare Klärung der Einflüsse von ER und ER ist unverändert von großer klinischer Bedeutung. Die Herstellung stabiler Zelllinien mit Überexpression von Reportergenkonstrukten und Rezeptoren kann dabei hilfreich sein. In dieser Arbeit wurde eine stabile Zelllinie mit Überexpression von ERβ etabliert, die unterschiedliche Wirkung von ER und ER wurden analysiert und die Effekte von zyklischer Dehnung auf Reportergenexpression unter der Kontrolle von mechanosensitiven responsiven Elementen wurden charakterisiert.
Infektionen des Schultergelenks gehen mit hoher Mortalität und Morbidität einher. Ihre Diagnostik und Therapie stellen häufig eine Herausforderung dar. Zudem ist - im Gegensatz zu Infektionen von Hüfte und Knie - die schulterspezifische Datenlage spärlich, es mangelt an evidenzbasierten Diagnose- und Therapiealgorithmen.
Diese Arbeit evaluiert unter anderem 1) den Erfolg der mehrzeitigen Implantation reverser Schulterendoprothesen zur Therapie von Infektionen des Glenohumeralelenks, 2) das schulterspezifische Keimspektrum, 3) den Stellenwert einer präoperativen Gelenkpunktion und 3) den Stellenwert einer Gelenkspunktion bei einliegendem Spacer.
Es handelt sich um eine retrospektive Studie mit prospektiver Datenerfassung an insgesamt 41 Patienten, welche zwischen 2007-2015 in der Orthopädischen Klinik König-Ludwig-Haus (Würzburg) die mehrzeitige Implantation einer Schulterendoprothese mit zwischenzeitlicher Implantation eines PMMA-Spacers auf Grund einer Infektion des Glenohumeralgelenks erhalten haben.
Die vorliegende Arbeit konnte zeigen, dass Infektionen des Glenohumeralgelenks gehäuft multimorbide, voroperierte Patienten betreffen. Die Haupterreger waren Cutibakterien und koagulasenegative Staphylokokken, insbesondere S. epidermidis. Diese Erreger sollten bei einer kalkulierten Antibiotikatherapie immer berücksichtigt werden.
Mikrobiologische Kulturen der präoperative Gelenkpunktion zeigten eine mäßige Sensitivität (58%) bei guter Spezifität (89%). Die Punktion kann somit keinen sicheren Infektionsausschluss bieten, ermöglicht im Falle eines Keimnachweises jedoch die gezielte antibiotische Therapie sowie die Anpassung der Antibiotika im Spacer und hat somit einen berechtigten Stellenwert in der Infektionsdiagnostik. Die Punktion bei einliegendem Spacer zeigte eine Sensitivität von 0%, ihre routinemäßige Durchführung sollte daher sehr kritisch hinterfragt werden.
Für die Therapie mit mehrzeitiger Implantation einer reversen Schulterendoprothese zeigte sich eine Infektionseradikationsrate von 87% und ein kumulatives infektionsfreies Überleben von 91% nach 128 Monaten. Die Funktionalität der Schulter blieb hinter der Funktion der Gegenseite zurück, war jedoch ausreichend um die eigenständige Versorgung und die Durchführung von Alltagsaktivitäten zu ermöglichen. Die große Mehrzahl der Patienten hatte keine/minimale Schmerzen.
Der zweizeitige Endoprothesenwechsel gilt als Goldstandard der Therapie periprothetischer Gelenkinfektionen. Ein sicherer Ausschluss einer Infektpersistenz ist für den Therapieerfolg essentiel. Es wurde der diagnostische Nutzen synovialer Marker zum Ausschluss einer Infektpersistenz bei einliegendem Kniegelenksspacer untersucht. Weder mikrobiologische noch zytologische Synovia-Analysen sind zum Ausschluss einer Infektpersistenz im Rahmen des zweizeitigen Knieendoprothesenwechsels geeignet.
Eine retrospektive Studie anhand von Patientenunterlagen zum Blutverlust, Transfusionshäufigkeit und postoperativen Komplikationen bei Knie-Endo-Prothesen mit systemischer Gabe von 500 mg Tranexamsäure kurz vor Eröffnung der Blutleere! 229 Patienten in der Studiengruppe, von denen 9 eine erlaubte Wiederholungsdosis erhalten hatten, wurden verglichen mit 231 Patienten, die keine Tranexamsäure erhalten hatten. 3 Patienten der Studiengruppe gegenüber 14 Patienten der Kontrollgruppe benötigten eine Bluttransfusion. In der Studiengruppe war der niedrigste postoperative Hb bei 71,2% der Patienten 10 g/dl oder höher. In der Kontrollgruppe war es nur bei 49,4% der Patienten der Fall. Durch die Gabe von Tranexamsäure konnte der durchschnittliche, berechnete Blutverlust bei Männern von etwa 1850 ml auf 1450 ml und bei Frauen von etwa 1300 ml auf etwa 1000 ml gesenkt werden. Die Unterschiede waren signifikant! Im beobachteten Kollektiv konnte keine Steigerung der Rate an Thrombosen oder thrombembolischen Ereignissen festgestellt werden. Das Antifibrinolytikum Tranexamsäure ist in dieser niedrigen Dosierung effektiv und scheint sicher zu sein.
Fragestellung: Epidemiologisch zeigt sich seit einigen Jahren ein Anstieg der Fallzahlen von Hüftpannenrevisionen. Zudem gewinnt die Lebensqualität der älter werdenden Bevölkerung zunehmend an Bedeutung. Eine Herausforderung für den behandelnden Chirurgen stellt insbesondere die Versorgung von größeren acetabulären Defekten dar. In dieser Studie wurden klinische und radiologische Ergebnisse von Hüftpfannenrevisionen, die durch ein kranial aufgesockeltes, längsovales Revisionspfannensystem versorgt wurden, untersucht. Ferner sollten mögliche Einflussfaktoren auf das Operationsergebnis ermittelt werden.
Methodik: In einer retrospektiven monozentrischen Studie wurden 155 Patienten, die im Zeitraum von 2009 bis 2016 mit Kranialpfannen versorgt wurden, nachuntersucht und Scores erhoben. Ein Unbedenklichkeitsbescheid der Ethikkommission der Universität Würzburg lag vor.
Ergebnisse: Das Follow-up lag im Durchschnitt bei 53 Monaten. Es wurden 9 AAOS I Defekte, 25 AAOS II Defekte, 100 AAOS III Defekte und 21 AAOS IV-Defekte nachuntersucht. Das kumulative Gesamtüberleben nach Kaplan-Meier betrug für diesen Follow-up Zeitraum 93,4% (CI 89,1-97,6%). Das revisionsfreie Überleben lag bei 84,0% (CI 77,9-90,1%). Es konnte eine signifikante Schmerzreduktion auf einer visuellen Analogskala belegt werden (5,9 präoperativ vs. 1,1 postoperativ). Der Harris-Hip-Score verbesserte sich hochsignifikant von 56,4 präoperativ auf 73,2 postoperativ. Ferner konnte in allen Dimensionen des Euroqols eine signifikante Verbesserung nachgewiesen werden. Der Gesundheitszustand der Patienten wurde anhand des SF-12 bestimmt. Als Ergebnis zeigte sich verglichen mit der Referenzpopulation eine leicht erniedrigte physische Gesundheit und eine geringgradig erhöhte psychische Gesundheit.
Im Rahmen dieser Arbeit konnten einige signifikante Einflussfaktoren auf das Operationsergebnis ermittelt werden: Das männliche Geschlecht ist mit einem signifikant besseren funktionellen Outcome assoziiert (HHS 87,7 vs. 74,2). Zudem gaben Männer auf der visuellen Analogskala eine signifikant geringere Schmerzintensität an (0,65 vs. 1,5). Darüber hinaus konnte belegt werden, dass höhergradige Defekttypen mit einem schlechteren funktionellen Outcome vergesellschaftet sind. Das funktionelle Outcome unterscheidet sich zudem signifikant in Abhängigkeit vom Alter der Patienten zum Zeitpunkt der Operation. Patienten unter 70 Jahren erreichten im Durchschnitt signifikant höhere Punktzahlen im Harris-Hip-Score (82,2 vs. 77,3). Septische Indikationen standen verglichen mit aseptischen Lockerungen ebenfalls in Assoziation mit einem schlechteren funktionellen Outcome (HHS 74,6 vs. 81,9).
Schlussfolgerung: Anhand der im Zuge dieser Arbeit ermittelten Überlebensrate, einer Verbesserung der Funktionalität im Hüftgelenk und einer nachgewiesenen Schmerzreduktion konnten gute kurz- bis mittelfristige Ergebnisse für das vorliegende Implantatsystem gezeigt werden. Als Einflussfaktoren auf das klinische Outcome konnten das Geschlecht, der zugrunde liegende Defekttyp, das Alter zum OP-Zeitpunkt und eine septische oder aseptische Indikation ermittelt werden.
Aufgrund des demographischen Wandels mit einer zunehmend älter werdenden Bevölkerung ist die Zahl der Knieprothesen und vor allem auch der Revisionsoperationen in den letzten Jahren deutlich gestiegen. Mit ebenfalls zunehmendem medizinischem Anspruch wurden die bestehenden Prothesenmodelle stetig weiter entwickelt.
Ziel der vorliegenden Studie war es, in einer prospektiv angelegten einfach verblindeten Studie zwei verschiedene Rotating-Hinge-Revisionsprothesen unterschiedlicher Hersteller miteinander zu vergleichen und sie auf ihr klinisches Outcome zu überprüfen.
Von Juni 2012 bis Mai 2013 wurden insgesamt 25 Patienten in die Studie aufgenommen und dann bis zu 12 Monate nach erfolgter Revisionsoperation nachuntersucht. Die Zuteilung der Prothesenarten erfolgte randomisiert, sodass 13 Patienten die Prothese „EnduRo“ und 12 Patienten die Prothese „Endo-Modell“ erhielten. Als Vergleichswerte wurden klinische Scores herangezogen.
In der Untersuchung zeigten beiden Vergleichsgruppen insgesamt eine deutliche Schmerzreduktion sowie eine Funktionsverbesserungen in allen erhobenen Scores. Zwar erreichten die Patienten der Gruppe „Endo-Modell“ etwas höhere Punktwerte, jedoch waren die Unterschiede im Vergleich zur Patientengruppe „EnduRo“ im durchgeführten Zweistichproben-T-Test nicht statistisch signifikant.
Trotz ständiger Weiterentwicklung der modernen Endoprothetik weist ein künstliches Gelenk auch heute eine begrenzte Haltbarkeit auf. Insbesondere bei der Versorgung jüngerer Patienten mit gestiegener Lebenserwartung wird häufig die Prothesenstandzeit aufgebraucht und ein komplizierter Revisionseingriff notwendig. Hierbei ist die aseptische Prothesenlockerung der häufigste Grund für den Austausch eines Implantates. Das Knochenmark des betreffenden Knochens enthält humane Mesenchymale Stammzellen (hMSC), die zur Aufrechterhaltung und Regeneration des Bindegewebes und des Knochens selbst beitragen. Ob diese Zellen z.B. durch ein Proliferations- oder Differenzierungsdefizit eine Rolle in der Ätiologie einer aseptischen Prothesenlockerung spielen, ist bisher nicht abschließend geklärt.
In der vorliegenden Arbeit wurde die regenerative Kapazität von hMSC von zwei Spendergruppen untersucht und verglichen: Als Wechselgruppe dienten Patienten, bei denen es zu einer aseptischen Prothesenlockerung und der Notwendigkeit eines Revisionseingriffes (Wechseloperation) gekommen war. Patienten, denen primär eine Hüft- oder Kniegelenksendoprothese eingesetzt wurde, bildeten die Kontrollgruppe. Zur Beurteilung der regenerativen Eigenschaften der hMSC wurde zum einen das Wachstums- und Alterungsverhalten in Zellkultur zum anderen die in vitro Differenzierungskapazität untersucht.
Die Knieendoprothetik verzeichnet in den letzten Jahren steigende Zahlen, was wiederum steigende Zahlen an Revisionen zur Folge hat. Diese Studie hat die Gründe für Revisionen ermittelt. Hierzu wurden 1245 Operationen gesammelt und ausgewertet. Instabilität, Infektion und aseptische Lockerungen stellen die Hauptgründe für eine Revisionsoperation dar. Es wurden zusätzlich verschiedene Zeiträume zwischen Primärimplantation und Revision unterteilt und die jeweiligen Gründe ermittelt.
In vorliegender Dissertation wurden die Langzeitergebnisse hinsichtlich der Lebensqualität der Patienten, die sich im Zeitraum 2002-2010 eine proximale Humerusfraktur zuzogen und mittels intramedullärer Drahtosteosynthese nach Kapandji im König Ludwig Haus in Würzburg operiert wurden, untersucht. Die Nachuntersuchung erfolgte mittels klinischer, radiologischer und sonografischer Untersuchung sowie durch Verwendung des DASH-Fragebogens, Constant Murley Score und der Visuellen Analogskala.
Der menschliche Körper besitzt Anpassungsmechanismen, die es ihm ermöglichen, sich an verschiedene Belastungssituationen anzupassen. Es gab in letzter Zeit mehrere Hinweise darauf, dass diese Mechanismen durch die Ausschüttung von Zytokinen, bzw. Myokinen durch die betroffenen Zellen selbst ausgelöst werden. In dieser Arbeit wurden die Serumkonzentration von Myostatin, Follistatin, Follistatin-like-3, Interleukin 6, Interleukin 8 und Klotho vor und nach einer kurzen körperlichen Belastung bestimmt. Dabei konnte allerdings keine signifikante Änderung der Konzentrationen nachgewiesen werden, was die Frage aufwirft, ob mesenchymales Gewebe, insbesondere Muskelgewebe, überhaupt über einen klassischen endokrinen Sekretionsmechanismus verfügt.
Autologous bone still represents today’s gold standard for the treatment of critical size bone defects and fracture non-unions despite associated disadvantages regarding limitations in availability, donor site morbidity, costs and efficacy. Bone tissue engineered constructs would present a promising alternative to currently available treatments. However, research on preclinical animal studies still fails to provide clinical applicable results able to allow the replacement of currently applied methods. It seems that the idea of bone tissue engineering, which has now been integral part of academic studies for over 30 years, got somehow stuck at an intermediate level, in between intense preclinical research and striven stages of initial clinical trial phases. A clear discrepancy exists between the number of studies with preclinical animal models for bone tissue engineering and the number of clinically approved bone tissue engineered constructs available to patients.
The aim of this thesis was hence to evaluate preclinical animal models for bone tissue engineering as well as the perception of scientists and clinicians towards these models. Moreover, the general role of bone tissue engineering and its clinical need assessed by scientists and surgeons was investigated. A survey was conducted questioning both scientific and clinical opinions on currently available study designs and researchers’ satisfaction with preclinical animal models. Additionally, a literature research was conducted, resulting in 167 papers from the last 10 years that report current designs of preclinical orthotopic animal studies in bone tissue engineering. Thereby, the focus lied on the description of the models regarding animal species, strain, age, gender and defect design. The outcome of the literature search was evaluated and compared to the outcome obtained from the survey.
The survey data revealed that both scientists and surgeons generally remain positive about the future role of bone tissue engineering and its step to clinical translation, at least in the distant future, where it then might replace the current gold standard, autologous bone. Moreover, most of the participants considered preclinical animal models as relevant and well developed but the results as not yet realizable in the clinics. Surgeons thereby demonstrated a slightly more optimistic perception of currently conducted research with animal models compared to scientists. However, a rather inconsistent description of present preclinical study designs could be discerned when evaluating the reported study designs in the survey and the papers of the literature search.
Indeed, defining an appropriate animal species, strain, age, gender, observation time, observation method and surgical design often depends on different indications and research questions and represents a highly challenging task for the establishment of a preclinical animal model. The existing lack of valid guidelines for preclinical testing of bone tissue engineering leads hence to a lack of well standardized preclinical animal models. Moreover, still existing knowledge gaps regarding aspects that affect the process of fracture healing, such as vascularization or immunological aspects, were found to hinder clinical translation of bone tissue engineered constructs.
Using literature review and survey, this thesis points out critical issues that need to be addressed to allow clinical translation of bone tissue engineered constructs. It can be concluded that currently existing study designs with preclinical animal models cannot live up to the claim of providing suitable results for clinical implementation. The here presented comprehensive summary of currently used preclinical animal models for bone tissue engineering reveals a missing consensus on the usage of models such as an apparent lack of reporting and standardization regarding the study designs described in both papers from the literature review and the survey. It thereby indicates a crucial need to improve preclinical animal models in order to allow clinical translation. Despite the fact that participants of the survey generally revealed a positive perception towards the use of bone tissue engineered constructs and affirmed the clinical need for such novel designs, the missing standardization constitutes a main weak point for the provision of reliable study outcome and the translational success of the models. The optimization of reproducibility and reliability, as well as the further understanding of ongoing mechanisms in bone healing in order to develop effective tissue engineered constructs, need to form the basis of all study designs. The study outcomes might then fulfill the requirements of maybe today's and hopefully tomorrow's aging population.
Physical interaction of skeletal precursors with multiple myeloma cells has been shown to suppress their osteogenic potential while favoring their tumor-promoting features. Although several transcriptome analyses of myeloma patient-derived mesenchymal stem cells have displayed differences compared to their healthy counterparts, these analyses insufficiently reflect the signatures mediated by tumor cell contact, vary due to different methodologies, and lack results in lineage-committed precursors. To determine tumor cell contact-mediated changes on skeletal precursors, we performed transcriptome analyses of mesenchymal stem cells and osteogenic precursor cells cultured in contact with the myeloma cell line INA-6. Comparative analyses confirmed dysregulation of genes which code for known disease-relevant factors and additionally revealed upregulation of genes that are associated with plasma cell homing, adhesion, osteoclastogenesis, and angiogenesis. Osteoclast-derived coupling factors, a dysregulated adipogenic potential, and an imbalance in favor of anti-anabolic factors may play a role in the hampered osteoblast differentiation potential of mesenchymal stem cells. Angiopoietin-Like 4 (ANGPTL4) was selected from a list of differentially expressed genes as a myeloma cell contact-dependent target in skeletal precursor cells which warranted further functional analyses. Adhesion assays with full-length ANGPTL4-coated plates revealed a potential role of this protein in INA6 cell attachment. This study expands knowledge of the myeloma cell contact-induced signature in the stromal compartment of myelomatous bones and thus offers potential targets that may allow detection and treatment of myeloma bone disease at an early stage.
Background:
Diese prospektiv randomisierte kontrollierte Single-Center-Studie untersucht das klinische Outcome 3 bis 12 Monate nach Implantation einer Hüfttotalendoprothese über den transglutealen lateralen Hüftzugang (Bauer).
Methods:
77 Patienten wurden in die transgluteale laterale Zugangsweg Kohorte randomisiert, wovon 50 Patienten die Studie beendeten. Alle Patienten wurden mit dem Harris Hip Score (HHS), dem Xtra Short Musculosceletal Functional Assesment Questionaire (XSFMA), dem 36-Item Short Form Survey (SF-36), einem Stepwatch™-Aktivitätsmonitor (SAM) und einem zeitabhängigen 25m-Gehtest bewertet. Mit Hilfe der postoperativen Standard- röntgenaufnahmen wurde die Inklination, Schaftpositionierung und das Offset bewertet.
Results:
Unsere Patienten verbesserten sich nach drei Monaten in Bezug auf den 25-m-Gehtest (prä-operativ 24 Sekunden, SD ± 6,07, 3 Monate 19,36 Sekunden, SD ± 3,80) und die vom Patienten geschätzte maximale Gehstrecke nicht signifikant (präoperativ 1187, SD ± 1809m und 12 Monate 5125m, SD ± 3867m). Mit der Stepwatch konnte ein postoperativer Anstieg der mittleren Gehzyklen pro Tag gemessen werden (präoperativ 4964, SD ± 2687, 3 Monate 5238, SD ± 2309, 12 Monate 5340, SD ± 1887). Der Bewegungsumfang verbesserte sich insbesondere für die Flexion (präoperativ 89°, 3 Monate 97,34°) und die Abduktion (präoperativ 25,31°, 3 Monate 37,45°) und blieb im weiteren Verlauf nahezu unverändert. Der HHS zeigte einen signifikanten Unterschied zwischen dem präoperativen Score (52,95, SD ± 15,69) und den Scores aller postoperativen Messzeitpunkte (3 Monate 88,40, SD ± 9,89, 6 Monate 89,05, SD ± 10,01, 12 Monate 91,43, SD ± 9,05, p <0,001). Gleiches gilt für den XSFMA-Funktionsindex, der sich zwischen dem präoperativen Score (40,50, SD ± 15,78) und den postoperativen Scores signifikant verbesserte (3 Monate 18,80, SD ± 16,12, 6 Monate 15,84, SD ± 15,39, 12 Monate 15,08, SD ± 16,33, p<0.001). Der SF-36-Score mit der Subgruppe der körperlichen Verfassung betrug präoperativ 25,62, SD ± 8,74 und war postoperativ ebenfalls signifikant höher (3 Monate 40,67, SD ± 10,01, 6 Monate 42,76, SD ± 9,59, 12 Monate 42,91, SD ± 11,86, p < 0,05), während der Subscore der psychischen Beurteilung sich stets auf konstantem Niveau befand (präoperativ 56,26, SD ± 9,23, 3 Monate 56,68, SD ± 8,23, 6 Monate 55,81, SD ± 7,22, 12 Monate 56,23, SD ± 6,92, p <0,05). Die Bewertung der postoperativen Röntgenbildgebung ergab eine durchschnittliche Pfanneninklination von 40,28 ° und einen femorales Offset von 38,1mm ohne Anzeichen von Migration.
Conclusion:
Zusammenfassend zeigen unsere Ergebnisse, dass sich bereits drei Monate nach Implantation einer Totalhüftendoprothese über den transglutealen lateralen Zugang die Lebensqualität, das Aktivitätsniveau, die Patientenzufriedenheit und der postoperative Bewegungsumfang signifikant verbessert haben.

Hibernoma is a rare benign lipomatous tumor showing differentiation of brown fatty tissue. To the author’s best knowledge, there is no known case of malignant transformation or metastasis. Due to their slow, noninfiltrating growth hibernomas are often an incidental finding in the third or fourth decade of life. The vast majority are located in the thigh, neck, and periscapular region. A diagnostic workup includes ultrasound and contrast-enhanced MRI. Differential diagnosis is benign lipoma, well-differentiated liposarcoma, and rhabdomyoma. An incisional biopsy followed by marginal resection of the tumor is the standard of care, and recurrence after complete resection is not reported. The current paper presents diagnostic and intraoperative findings of a hibernoma of the upper arm and reviews similar reports in the current literature.
Background
Radiographic imaging is an important tool to assess osteoarthritis (OA). Lateral compartment osteoarthritis (valgus OA) usually starts with cartilage degeneration along the posterior aspect of the lateral femoral condyle. There is evidence that the posterior-anterior (PA)-flexed view is more sensitive when diagnosing early stages of valgus OA compared to the anterior-posterior (AP) view. The current paper analyzes the value of the PA-flexed view for patients scheduled for total knee arthroplasty (TKA).
Methods
Radiographs of 134 valgus knees were assessed prior to TKA. The minimal joint space width (minJSW) was measured on AP and PA-flexed views. The extent of mechanical deformity was measured on hip to ankle standing films.
Results
49 (36.6%) AP views showed Kellgren and Lawrence (K/L)-grade 4 osteoarthritis in the lateral compartment, 82 (63.4%) showed grade 3 or less. The PA-flexed view resulted in an increased K/L-grading to grade 4 for 53 knees (62.4%) that were considered grade 3 or less on standard AP-radiographs. There was a significant differences between lateral minJSW on AP and PA-flexed view for patients with up to 10 degrees of mechanical valgus deformity (p < 0.001), as well as 11 to 15 degrees of mechanical deformity (p = 0.021). Only knees with severe deformity of more than 15 degrees did not show a difference in minJSW between PA-flexed view and AP view (p = 0.345).
Conclusions
The PA-flexed view is superior to the standard AP view in quantifying the extent of valgus OA in patients with zero to fifteen degrees of valgus deformity. It is recommended for the initial assessment of patients with valgus osteoarthritis and better documents the extent of osteoarthritis prior to TKA.
Wirkung von WISP-3 auf dedifferenzierte Chondrozyten und mesenchymale Stammzellen
Degenerative Gelenkerkrankungen (Arthrosen) sind einige der häufigsten Ursachen für eine Vorstellung und Beratung in der allgemeinmedizinischen Praxis. Der Großteil der über 65-jährigen ist davon betroffen, wenn auch die Symptome in ihrer Ausprägung stark variieren können. Eine ursächliche Therapie ist bisher nicht bekannt. Es wird symptomatisch behandelt und versucht, die Symptome zu lindern und den Progress der Erkrankung zu verlangsamen. Dabei entstehen dem Gesundheitssystem durch die hohe Prävalenz nicht unerhebliche Kosten für Medikamente, Physiotherapie, Operationen und Reha-Aufenthalte.
Arthrosen beruhen auf Knorpelschädigungen, die ohne Therapie immer weiter fortschreiten können und zu Schmerzen, Bewegungseinschränkung und Verformung des Gelenks führen können. Diese Knorpelschädigungen können auf verschiedenen Ursachen beruhen wie z.B. zu hoher Belastung (bei Adipositas), Traumata oder Erkrankungen des Skelettsystems. Neue Behandlungsansätze werden mit zunehmendem Durchschnittsalter der Bevölkerung immer wichtiger werden.
Ein Ansatzpunkt ist die weitere Erforschung von Proteinen wie z.B. WISP-3, die einen positiven Effekt auf die Knorpelhomöostase bewirken. Die Bedeutung von WISP-3 fiel bei der Erforschung der Progressiven Pseudorheumatoiden Dysplasie auf, einer seltenen Gelenkerkrankung, die v.a. bei Kindern auftritt. Dabei treten auf Grund von Mutationen von WISP-3 Störungen in der Knorpelhomöostase und Gelenkarchitektur auf, die sogar eine Therapie mittels Gelenkersatz notwendig machen können.
In unseren Versuchen zeigte sich, dass WISP-3 in Verbindung mit Wachstumsfaktoren eine positive Wirkung auf das Wachstum und die Differenzierung von dedifferenzierten Chondrozyten aufweist. Somit werden neue Forschungsansätze in der Arthrosetherapie aufgezeigt. Für die weitere Forschung kann auch auf mesenchymale Stammzellen zurückgegriffen werden, die vielversprechende Aussichten besonders im Tissue Engineering bieten.
Für die gesamte CCN-Familie ergeben sich noch weitere vielfältige Forschungsmöglichkeiten, wie z.B. in der Therapie von Mamma-, Gebärmutter- oder Nierenzellkarzinomen.
Erstellung und Auswertung einer Tumordatenbank in der Orthopädischen Klinik König-Ludwig-Haus

Die vorliegende Arbeit befasst sich mit der Analyse eines Patientenkollektivs von 771 Tumorpatienten, welche im Zeitraum von September 2009 bis 31.12.2014 im interdisziplinären muskuloskelettalen Tumorboard des Comprehensive Cancer Center Mainfranken der Universitätsklinik Würzburg vorgestellt wurden. Es wurde eine retrospektive Betrachtung dieser Patienten durchgeführt, um das behandelte Patientenkollektiv zu typisieren und einen Überblick über Art und Anzahl der betreuten Patienten zu erhalten.
Osteozyten stehen vermehrt im Fokus als wesentliche Regulatoren der Knochenmineralisierung. Das ähnlich einem neuronalen Netzwerk aufgebaute lakunokanalikuläre Netzwerk der Osteozyten breitet sich im Knochen in drei Ebenen aus. Es wurde in dieser Arbeit ein 3D-Kollagengel-Modell verwendet und dort die Osteoblasten- bzw. Osteozytenzelllinien MLO-A5 und MLO-Y4, sowie humane mesenchymale Stammzellen aus Hüftköpfen eingebettet. Es wurden die optimalen Kulturbedingungen entwickelt und die Zellen über mehrere Wochen kultiviert, beobachtet und mit dem herkömmlichen 2D-Kulturmodell verglichen. MLO-A5 und MLO-Y4 bilden die zelltypischen Zellfortsätze. Die Gele kontrahieren, wenn hMSC und MLO-A5 eingebettet sind, mit MLO-Y4 zeigt sich über den gesamten Kultivierungszeitraum keinerlei Kontraktion der Kollagengele. Die Zellen wurden zudem osteogen differenziert und mit FGF23 und Klotho stimuliert. Es ergaben sich erste Hinweise auf eine FGF23 / Klotho-abhängige Inhibierung der lokalen Mineralisierung in osteogen differenzierten MLO-A5.
Es konnten einige osteogene Marker durch PCR und in den histologischen Schnitten mittels Antikörperfärbungen nachgewiesen werden, eindeutige Expressionsmuster und deren zeitliche Verläufe im Vergleich der osteogenen Differenzierungen und Zugabe von FGF23 und Klotho sind allerdings noch nicht identifizierbar und bedürfen womöglich höherer Fallzahlen und weiterer Untersuchungsmethoden. Insgesamt gesehen erweist sich das System aber als einfach und mit niederschwellig erreichbaren Methoden und Materialien durchzuführen.
Objectives
The long head of the biceps (LHB) is often resected in shoulder surgery and could therefore serve as a cell source for tissue engineering approaches in the shoulder. However, whether it represents a suitable cell source for regenerative approaches, both in the inflamed and non-inflamed states, remains unclear. In the present study, inflamed and native human LHBs were comparatively characterized for features of regeneration.
Methods
In total, 22 resected LHB tendons were classified into inflamed samples (n = 11) and non-inflamed samples (n = 11). Proliferation potential and specific marker gene expression of primary LHB-derived cell cultures were analyzed. Multipotentiality, including osteogenic, adipogenic, chondrogenic, and tenogenic differentiation potential of both groups were compared under respective lineage-specific culture conditions.
Results
Inflammation does not seem to affect the proliferation rate of the isolated tendon-derived stem cells (TDSCs) and the tenogenic marker gene expression. Cells from both groups showed an equivalent osteogenic, adipogenic, chondrogenic and tenogenic differentiation potential in histology and real-time polymerase chain reaction (RT-PCR) analysis.
Conclusion
These results suggest that the LHB tendon might be a suitable cell source for regenerative approaches, both in inflamed and non-inflamed states. The LHB with and without tendinitis has been characterized as a novel source of TDSCs, which might facilitate treatment of degeneration and induction of regeneration in shoulder surgery.
Background:
Sarcopenic obesity (SO) is characterized by a combination of low muscle and high fat mass with an additive negative effect of both conditions on cardiometabolic risk. The aim of the study was to determine the effect of whole-body electromyostimulation (WB-EMS) on the metabolic syndrome (MetS) in community-dwelling women aged ≥70 years with SO.
Methods:
The study was conducted in an ambulatory university setting. Seventy-five community-dwelling women aged ≥70 years with SO living in Northern Bavaria, Germany, were randomly allocated to either 6 months of WB-EMS application with (WB-EMS&P) or without (WB-EMS) dietary supplementation (150 kcal/day, 56% protein) or a non-training control group (CG). WB-EMS included one session of 20 min (85 Hz, 350 µs, 4 s of strain–4 s of rest) per week with moderate-to-high intensity. The primary study endpoint was the MetS Z-score with the components waist circumference (WC), mean arterial pressure (MAP), triglycerides, fasting plasma glucose, and high-density lipoprotein cholesterol (HDL-C); secondary study endpoints were changes in these determining variables.
Results:
MetS Z-score decreased in both groups; however, changes compared with the CG were significant (P=0.001) in the WB-EMS&P group only. On analyzing the components of the MetS, significant positive effects for both WB-EMS groups (P≤0.038) were identified for MAP, while the WB-EMS group significantly differed for WC (P=0.036), and the WB-EMS&P group significantly differed for HDL-C (P=0.006) from the CG. No significant differences were observed between the WB-EMS groups.
Conclusion:
The study clearly confirms the favorable effect of WB-EMS application on the MetS in community-dwelling women aged ≥70 years with SO. However, protein-enriched supplements did not increase effects of WB-EMS alone. In summary, we considered this novel technology an effective and safe method to prevent cardiometabolic risk factors and diseases in older women unable or unwilling to exercise conventionally.
Einführung
Der laterale Zugang nach Bauer ist weiterhin einer der etabliertesten Standardzugänge für die Hüftendoprothetik in Deutschland und weltweit. Im Zuge der Entwicklung in Richtung „Fast-Track“ Endoprothetik gewinnt die frühe postoperative Phase zunehmend an Bedeutung.
Material und Methoden:
Die vorliegende Arbeit untersucht die Ergebnisse des lateralen OP-Zugangs bis sechs Wochen postoperativ, die in einer übergeordneten prospektiv randomisierten Studie zum Vergleich des lateralen mit dem anterioren MIS-Zugangs erhoben wurden. Diese Studie wurde bereits international publiziert. In der vorliegenden Arbeit sollte ermittelt werden, welche Änderungen sich in den Outcome-Kriterien Aktivität, Funktion und Lebensqualität durch den lateralen Zugang bis sechs Wochen postoperativ ergeben. Hierfür wurden neben den etablierten Scores HHS, AP, SF-36 und PHQ-D der Aktivitätsscore TWB sowie der Funktionsscore XSMFA-D erstmalig in der frühen postoperativen Phase eingesetzt.
Ergebnisse:
Maßgeblich für die Bewertung der Ergebnisqualität sind neben den Ergebnissen der Scores auch die intra- und postoperativen Komplikationen, welche auf einem vergleichsweise niedrigen Niveau waren. 7,5 % der Patienten mit lateralem Zugang zeigten Hinweise für eine Schwächung der Hüftabduktoren, die häufig mit dem transglutealen Zugangsweg in Verbindung gebracht wird. Der laterale Zugang konnte bis sechs Wochen postoperativ und auch im Gesamtkontext der Studie nicht mit einem erhöhten peri- und postoperativen Komplikationsrisiko in Zusammenhang gebracht werden.
Das Outcome in Aktivität, Funktion und Lebensqualität zeigte im prä- und postoperativen W6-Vergleich signifikante Zunahmen in den Scores HHS, XSMFA-D, SF-36 und AP mit hohen Korrelationen untereinander und jeweils anhand der Score-Outcome-Kriterien und im Literaturvergleich guten bis sehr guten Ergebnissen. Dies trifft ebenfalls auf den Einsatz des StepWatch™ präoperativ und im weiteren Verlauf der Studie zu. Im Gesamtkontext der bereits publizierten übergeordneten Studie konnten jedoch für den MIS-Zugang in einzelnen Scores bessere Ergebnisse in den Outcome-Kategorien erzielt werde.
Schlussfolgerungen:
Die Ergebnisse des erstmalig früh postoperativ eingesetzten TWB lassen die Eignung des Scores zur validen Datenerhebung unter diesen Bedingungen fraglich erscheinen. Eine evtl. Revalidierung des TWB sollte deshalb, bei Bestätigung der vorliegenden Beobachtungen im weiteren klinischen Gebrauch in Betracht gezogen werden. Auch der PHQ-D konnte seine in der Literatur beschriebene Eignung zur postoperativen Datenerhebung von psychischen Komorbitäten bei chirurgischen Patienten bis W6 nicht unterstreichen.
Im Gegensatz zu TWB und PHQ-D hat sich der erstmalige Einsatz des XSMFA-D im frühen Nachuntersuchungsintervall bewährt. Es konnten Verbesserungen für Funktion und Beeinträchtigung sechs Wochen postoperativ bereits auf vergleichbarem Niveau wie zu den späteren Nachuntersuchungen ermittelt werden.
Schlussfolgern lässt sich, dass die frühe postoperative Phase eine in der klinischen Bedeutung und postoperativen Ergebnisbeurteilung zunehmend wichtige Zwischenstation darstellt, bei der die bis dort in den Outcome-Kriterien Aktivität, Funktion und Lebensqualität auftretenden signifikanten Verbesserungen auch Rückschlüsse auf die weitere Prognose und Outcome-Entwicklung zulassen. Der in der vorliegenden Arbeit speziell untersuchte laterale OP-Zugang konnte im Gesamtkontext der übergeordneten prospektiven randomisierten Studie die Ergebnisqualität des MIS-Verfahrens sechs Wochen postoperativ nicht erreichen. Die Ergebnisqualität des lateralen Zugangs zur Behandlung der Coxarthrose ist aber anhand der Score-Outcome-Kriterien und den verglichenen Literaturergebnissen bis sechs Wochen postoperativ dennoch als gut zu bewerten. Vorteilhaft und von Bedeutung sind diese Erkenntnisse insbesondere für diejenigen Patienten, welche nicht für das MIS-Verfahren, sondern den lateralen OP-Zugang in Frage kommen.
Die Knieendoprothetik ist ein etabliertes Verfahren zur Behandlung der symptomatischen Gonarthose, mit welchem gute Ergebnisse erzielt werden können. Dennoch kommt es in 10-20% zu einer Beschwerdepersistenz und die aseptische Lockerung der Prothesen stellt weiterhin ein Problem dar.
Als mögliche Ursache hierfür wird die unzureichend abgebildete Anatomie und Biomechanik diskutiert.
Die konvex gekrümmten Femurkondylen sind nicht symmetrisch. Die Krümmungsradien nehmen von dorsal nach ventral zu. Zudem ist die mediale Gelenkfläche des Tibiaplateaus nach kranial konkav und die laterale nach kranial konvex zulaufend. Die Instabilität wird durch das vordere Kreuzband gesichert. Bei der Bewegung des Kniegelenkes kommt es zu einer Roll/Gleitbewegung zwischen Femur und Tibia. Bisher gibt es nur wenige Knieendoprothesen, die das natürliche Roll-Gleit-Verhalten des natürlichen Kniegelenkes nachempfinden. Die Aequos G1 Endoprothese berücksichtigt die Besonderheiten (mediolaterale Asymmetrie) und soll so die natürliche Kinematik besser nachempfinden.
Ziel dieser Arbeit war es, Daten vom klinischen und radiologischen Ergebnis nach Implantation der Prothese innerhalb eines Jahres zu erheben. Es wurde Wert gelegt auf die Erfassung von funktionellem Outcome, gesundheitsbezogener Lebensqualität und Probleme der Patella wie dem vorderen Knieschmerz.
Im Zeitraum von 2010 bis 2011 wurden 26 Patienten in einer prospektiven, klinischen Verlaufsstudie erfasst. Es erfolgte eine Untersuchung präoperativ, eine vor Entlassung aus dem Krankenhaus und nach 6 und 12 Monaten. Die Patienten wurden mittels standardisierter Fragebögen befragt und Daten erhoben zu Knee Society Score (KSS), Oxford Knee Score (OKS), Visuelle Analogskala Schmerz (VAS), Kujala- Score, Turba- Score und SF-36. Zudem erfolgten eine klinische Untersuchung und eine standardisierte radiologische Diagnostik.
Das mittlere Alter der Patienten zum Zeitpunkt der Versorgung lag bei 68,77 Jahren (Min. 55 – Max. 83). Der Frauenanteil lag bei 76,92%, der durchschnittliche BMI betrug 31,48kg/m². Insgesamt konnten 22 Patienten nach 12 Monaten inkludiert werden. Eine Patientin war zum Zeitpunkt der Nachuntersuchung verstorben, eine Patientin erschien nicht zur Nachuntersuchung nach 6 Monaten, zwei Patienten erhielten einen sekundären Retropatellarersatz und schieden aus der Studie aus. Bei einer Patientin zeigte sich 12 Monate postoperativ eine Lockerung des Tibiaplateaus, es erfolgte ein Wechsel der tibialen Komponente. Eine Patientin klagte nach 6 Monaten überstarke, wirbelsäulenbedingte Beinschmerzen.
Der mittlere Bewegungsumfang besserte sich nicht signifikant. Er betrug präoperativ 106,68° (±22,86°) und nach 12 Monaten 113,18° (±8,67°). Sowohl die VAS, als auch der KSS, der OKS und der Kujala-Score waren 12 Monate postoperativ hochsignifikant besser als präoperativ. Die berechnete körperliche Summenskala des SF-36 war ebenfalls hoch signifikant besser als präoperativ. Die Daten der psychischen Summenskala waren signifikant besser als präoperativ.
Der subjektive Teil des Turba-Scores war hoch signifikant besser als präoperativ. Im objektiven Teil zeigte sich keine signifikante Besserung.
Insgesamt zeigten sich gute postoperative Ergebnisse nach Implantation der Aequos G1 Endoprothese nach einem Jahr mit signifikanter Verbesserung der Funktions- und Schmerzscores sowie der individuellen Gesundheitswahrnehmung. Im Vergleich mit den Ergebnissen der verfügbaren Literatur, scheint die Prothese nicht unterlegen.
Abzuwarten bleibt, ob sich die guten Ergebnisse langfristig bestätigen.
Ziel der vorliegenden Studie war, die Überlebensrate der Hybrid-PFC-Knieprothese im Langzeit-verlauf darzustellen.
Insgesamt wurden 91 Patienten erfasst (50 mit klinischer und radiologischer Untersuchung und 41 anhand der Fragebögen). Es handelt sich um 78 Frauen und 13 Männer mit einem durchschnittlichen Alter von 79,6 Jahren zum Nachuntersuchungszeitpunkt (März bis Juni 2004).
Der Nachuntersuchungszeitraum umfasst 12 - 16 Jahre nach Implantation.
Ausgewertet wurden die folgenden standartisierten Fragebögen: WOMAC, SMFA-D, KSS. Die radiologische Untersuchung erfolgte gemäß KSRESS.
Die Überlebenszeitanalyse beträgt 88% nach 15 Jahren. In diesem Zeitraum wurden 10 Revisionen erfasst.
WOMAC mit insgesamt guten Ergebnissen bezüglich Schmerz (3,2), Steife (3,6), Funktion (4,1) und Gesamtscore (3,7). SMFA-D mit eher mittelmäßigen Werten von 42,8 im Funktionsindex und 42,4 im Beeinträchtigungsindex und der Knee Society Score (KSS) mit einem mäßigen Ergebnis von 78,8 im Kniescore und einem schlechten Wert 53,6 im Funktionsscore.
Die radiologische Auswertung gemäß KSRESS zeigte Aufhellungslinien > 2mm im Bereich D_16_1 1 (Femurschild Position 1) und größere Osteolysen (1 x 8mm und 1 x 10mm) im Bereich D16_4_2 (dorsales Tibiaplateau seitlich), jedoch keine Komponentenlockerung.
Die vorliegende Studie zeigt mittelgradige Ergebnisse in den erhobenen Scores bei höherem Durchschnittsalter der erhobenen Studienteilnehmer gegenüber Vergleichs-studien. Die Langzeit-Überlebensrate von 88% zeigt sich ebenbürtig gegenüber anderen Verfahren. Die kürzere OP-Zeit beim Hybridverfahren ist als Vorteil zu sehen. Somit bietet die Hybridtechnik eine gute zusätzliche chirurgische Option in der Knieendoprothetik.
Zusammenfassend kann hier berichtet werden das die PFC-Modular Hybrid-Knieendo-prothese insgesamt eine gute Haltbarkeit über einen Zeitraum bis zu 16 Jahre bietet. Die
eher mäßigen Scores der Fragebögen wurden vermutlich durch das hohe Alter der Patienten beeinflußt.
Im seltenen Krankheitsbild der Hypophosphatasie (HPP) treten aufgrund der Fehlfunktion der Gewebe-unspezifischen Alkalischen Phosphatase (tissue-nonspecific alkaline phosphatase, TNAP) skelettale und dentale Symptome in sehr variabler Ausprägung auf. Der vorzeitige Verlust von Milchzähnen ist das zahnmedizinische Leitsymptom und in vielen Fällen ein erstes Anzeichen dieser Erkrankung. In dieser Arbeit wurde ein in vitro Modell der HPP etabliert und der Fokus auf die dentalen Aspekte dieser Erkrankung gelegt. Hierzu wurden mesenchymale Stammzellen (MSCs) aus Bereichen analysiert, die bei einer Erkrankung von dieser Mineralisierungsstörung betroffen sind. Es wurden dentale Stammzellen aus der Pulpa (dental pulp stem cells, DPSCs) und dem parodontalen Ligament (periodontal ligament stem cells, PDLSCs) isoliert und im Vergleich zu Stammzellen aus dem Knochenmark (bone marrow mesenchymal stem cells, BMSCs) charakterisiert. Um den Einfluss der Spendervariabilität zu reduzieren, wurden aus dem gesamten dentalen Probenmaterial nur vollständige Probenpaare aus DPSCs und PDLSCs von 5 Spendern für die vergleichenden Analysen verwendet. Die dentalen MSCs konnten somit paarweise direkt miteinander verglichen werden.
DPSCs gelten seit ihrer Entdeckung von Gronthos et al. im Jahr 2000 als geeignete Quelle für die Stammzellgewinnung mit vielversprechenden Anwendungsmöglichkeiten im Bereich des Tissue Engineering und der regenerativen muskuloskelettalen Medizin. PDLSCs sind aufgrund der parodontalen Problematik der HPP von besonderem Interesse in dieser Arbeit. Die Isolation von Stammzellen aus Pulpa und PDL konnte mit dem Nachweis der sogenannten Minimalkriterien für MSCs bestätigt werden.
In diesem durch Enzyminhibition mit Levamisol induzierten in vitro Modell der HPP wurde die TNAP-abhängige Genexpression, die Enzym-Aktivität und das osteogene Differenzierungspotenzial an diesen drei Mineralisierungs-assoziierten MSCs untersucht. Die erweiterte Genexpressionsanalyse in Kooperation mit der Core Unit Systemmedizin der Universität Würzburg mit einer RNA-Sequenzierung der PDLSCs ergab interessante Einblicke in die differentielle Genexpression nach der TNAP-Inhibition während der osteogenen Differenzierung und Ansatzpunkte für weitere Analysen. Die beobachteten Genregulationen waren nach dem derzeitigen Verständnis pathologischer Zusammenhänge nachvollziehbar und simulierten in vitro HPP-relevante Signalwege repräsentativ. Insbesondere die signifikante Genregulation von P2X7 und DMP1, sowie Zusammenhänge aus dem Wnt-Signalweg zeigen hinsichtlich der dentalen Aspekte der HPP neue Ansatzpunkte auf. Die erhöhte P2X7-Expression in diesem in vitro HPP-Modell scheint mit der Parodontitis-Problematik der HPP zu korrelieren und verdeutlicht unter anderem die multifaktorielle Ätiologie und Pathogenese der Parodontitis. Die Tatsache, dass die experimentellen Beobachtungen in Einklang mit dem klinischen Bild der HPP gebracht werden können, bestätigt die Relevanz des hier etablierten in vitro Modells. Zusammenfassend konnten anhand dieses in vitro Modells der HPP neue Aspekte aufgedeckt werden, die nicht nur im Hinblick auf die dentale Problematik der HPP aufschlussreich sind.
Background: Mesenchymal stem cells (MSCs) and their chondrogenic differentiation have been extensively investigated in vitro as MSCs provide an attractive source besides chondrocytes for cartilage repair therapies. Here we established prototype foamyviral vectors (FVV) that are derived from apathogenic parent viruses and are characterized by a broad host range and a favorable integration pattern into the cellular genome. As the inflammatory cytokine interleukin 1 beta (IL1β) is frequently present in diseased joints, the protective effects of FVV expressing the human interleukin 1 receptor antagonist protein (IL1RA) were studied in an established in vitro model (aggregate culture system) of chondrogenesis in the presence of IL1β.
Materials and Methods: We generated different recombinant FVVs encoding enhanced green fluorescent protein (EGFP) or IL1RA and examined their transduction efficiencies and transgene expression profiles using different cell lines and human primary MSCs derived from bone marrow-aspirates. Transgene expression was evaluated by fluorescence microscopy (EGFP), flow cytometry (EGFP), and ELISA (IL1RA). For evaluation of the functionality of the IL1RA transgene to block the inhibitory effects of IL1β on chondrogenesis of primary MSCs and an immortalized MSC cell line (TERT4 cells), the cells were maintained following transduction as aggregate cultures in standard chondrogenic media in the presence or absence of IL1β. After 3 weeks of culture, pellets were harvested and analyzed by histology and immunohistochemistry for chondrogenic phenotypes.
Results: The different FVV efficiently transduced cell lines as well as primary MSCs, thereby reaching high transgene expression levels in 6-well plates with levels of around 100 ng/ml IL1RA. MSC aggregate cultures which were maintained in chondrogenic media without IL1β supplementation revealed a chondrogenic phenotype by means of strong positive staining for collagen type II and matrix proteoglycan (Alcian blue). Addition of IL1β was inhibitory to chondrogenesis in untreated control pellets. In contrast, foamyviral mediated IL1RA expression rescued the chondrogenesis in pellets cultured in the presence of IL1β. Transduced MSC pellets reached thereby very high IL1RA transgene expression levels with a peak of 1087 ng/ml after day 7, followed by a decrease to 194 ng/ml after day 21, while IL1RA concentrations of controls were permanently below 200 pg/ml.
Conclusion: Our results indicate that FVV are capable of efficient gene transfer to MSCs, while reaching IL1RA transgene expression levels, that were able to efficiently block the impacts of IL1β in vitro. FVV merit further investigation as a means to study the potential as a gene transfer tool for MSC based therapies for cartilage repair.
Epidermal growth factors (EGFs) e.g. EGF, heparin-binding EGF and transforming growth factor alpha and their receptors e.g. EGFR and ErbB2 control proinflammatory signaling and modulate proliferation in bone marrow stromal cells (BMSC). Interleukin-6 and interleukin-8 are EGF targets and participate in the inflammatory phase of bone regeneration via non-canonical wnt signaling. BMSC differentiation is also influenced by mechanical strain-related activation of ERK1/2 and AP-1, but the role of EGFR signaling in mechanotransduction is unclear. We investigated the effects of EGFR signaling in telomerase-immortalized BMSC, transfected with a luciferase reporter, comprising a mechanoresponsive AP1 element, using ligands, neutralizing antibodies and EGFR inhibitors on mechanotransduction and we found that EGF via EGFR increased the response to mechanical strain. Results were confirmed by qPCR analysis of mechanoresponsive genes. EGF-responsive interleukin-6 and interleukin-8 were synergistically enhanced by EGF stimulation and mechanical strain. We show here in immortalized and primary BMSC that EGFR signaling enhances mechanotransduction, indicating that the EGF system is a mechanosensitizer in BMSC. Alterations in mechanosensitivity and -adaptation are contributors to age-related diseases like osteoporosis and the identification of a suitable mechanosensitizer could be beneficial. The role of the synergism of these signaling cascades in physiology and disease remains to be unraveled.

Beim Multiplen Myelom handelt es sich um eine klinisch sehr heterogene Erkrankung, bei der eine genaue Prognoseabschätzung schwierig ist. In den letzten Jahrzehnten haben sich mehrere klinische, laborchemische sowie genetische Parameter als Risikofaktoren etabliert. Das Cystein-rich-Protein 61 (CYR61) ist ein Signalprotein, das bei verschiedensten Prozessen im menschlichen Körper eine Rolle spielt. Im Knochen ist CYR61 zuständig für die Angiogenese, die Proliferation und Migration mesenchymaler Stammzellen sowie für die Differenzierung von Osteoblasten und Endothelzellen. Im Zusammenhang mit dem Multiplen Myelom konnte in Studien gezeigt werden, dass CYR61 dort in signifikant höheren Konzentrationen vorliegt und hohe CYR61-Werte bei Myelom-Patienten mit einer besseren Prognose und einem längeren Überleben einherzugehen scheinen.
In der vorliegenden Dissertation, wurde die CYR61-Konzentration im peripheren Blut bei Myelom-, Osteoporose-Patienten und gesunden Probanden gemessen. Es zeigten sich signifikant höhere CYR61-Werte in der Gruppe der Myelom-Patienten im Vergleich zur Gruppe der Osteoporose-Patienten. Hohe CYR61-Konzentrationen in der Myelom-Gruppe waren assoziiert mit günstigen Prognosefaktoren wie einem Hämoglobin-Wert > 10 g/dl, einem normwertigen Serum-Albumin, einer niedrigen Laktatdehydrogenase, einer geringen Konzentration von Immunglobulinen im Serum und einer normwertigen free light chain ratio. Patienten in "complete response" oder "very good partial response" hatten signifikant höhere CYR61-Werte. Eine Therapie mit immunmodulatorischen Medikamenten ging mit höheren CYR61-Konzentrationen einher. Der Einsatz von Proteasom-Inhibitoren war dagegen mit niedrigeren CYR61-Werten assoziiert. In der Gruppe der Osteoporose-Patienten sowie bei den gesunden Probanden ergaben sich signifikant höhere CYR61-Konzentrationen bei fortgeschrittenem Alter.
Zusammenfassend lassen die Ergebnisse vermuten, dass eine hohe CYR61-Konzentration im Serum von Myelom-Patienten im Zusammenhang mit einer geringeren Krankheitsaktivität bzw. mit einer stattfindenden Regeneration des Knochens steht. Hohe CYR61-Werte bei Myelom-Patienten waren ausschließlich mit günstigen Risiko- und Prognosefaktoren assoziiert. Eine gesteigerte Ausschüttung von CYR61 scheint für einen anabolen Knochenstoffwechsel zu sprechen. Passend dazu zeigten sich bei Osteoporose-Patienten signifikant niedrigere CYR61-Werte. In Bezug auf die durchgeführten Therapien beim Multiplen Myelom ist ein komplexer Zusammenhang zwischen Wirkungsweise der Medikamente auf das Knochenmikromilieu und der Regulation der CYR61-Produktion anzunehmen. CYR61 sollte als sensibler Parameter angesehen werden, dessen Aussage sich mit dem Verlauf der Erkrankung und unter Berücksichtigung der durchgeführten Therapien ändern kann. Beim Einsatz von Proteasom-Inhibitoren kann CYR61 möglicherweise als Parameter für das Ansprechen der Therapie genutzt werden.
In einer randomisierten und doppelblinden Studie wurde der Effekt der frühen postoperativen Akupunktur auf die passive Beugefähigkeit im Kniegelenk und Schmerzreduktion nach Knieprothesenimplantation untersucht. Der Untersuchungszeitraum lag zwischen dem 2. und 6. postoperativen Tag. In der Auswertung der Studie zeigte sich eine Verbesserung der Beugefähigkeit unter Verringerung der Ruheschmerzintensität in der Verum-Akupunkturgruppe. Die statistische Auswertung ergab hier jedoch keine ausreichenden signifikanten Unterschiede zwischen den Vergleichsgruppen.
Retrospektive prä- und postoperative radiologische Datenauswertung patientenspezifischer Kniegelenksendoprothetik – ConforMIS iTotal® CR, iDuo® und iUni® G2.
Vergleich und Diskussion der Daten hinsichtlich mechanischer Beinachse, Patellahöhe und -ausrichtung und Komponentendimensionierung und -ausrichtung.
Die Implantation von Kniegelenks-Totalendoprothesen stellt heutzutage einen Routineeingriff dar, ist allerdings mit postoperativen Schmerzen und einer langwierigen Rehabilitation verbunden. In den letzten Jahren haben alternative Heilmethoden wie etwa die Traditionelle Chinesische Medizin eine zunehmende Verbreitung erfahren und speziell für die Akupunktur konnte auch eine schmerzlindernde Wirkung gezeigt werden. Inwieweit eine Akupunktur-behandlung in der postoperativen Situation das funktionelle Ergebnis verbessern kann ist unklar. Im Rahmen dieser Studie wurde deshalb der Einfluss einer Akupunktur auf das funktionelle Ergebnis zwischen dem zweiten postoperativen Tag und drei Monate nach der Operation untersucht. Dazu wurde eine randomisierte, kontrollierte und dreifach verblindete Monozenterstudie mit 60 Teilnehmern durchgeführt. Die Kontrollgruppe erhielt dabei die postoperative Standardtherapie, während die Interventionsgruppe vom 2. bis 6. Tag nach der Operation einmal täglich zusätzlich akupunktiert wurde. Im gleichen Zeitraum wurde eine dritte Studiengruppe, die Placebogruppe, scheinakupunktiert. Vom 2. bis 6. Tag nach der Operation sowie drei Monate später wurden die postoperativen Schmerzintensitäten abgefragt. Weiterhin wurden die maximal mögliche, passive Kniebeugung und die maximal mögliche Kniebelastung drei Monate nach der Operation ermittelt und das funktionelle Ergebnis der Operation mittels Fragebögen durch die Patienten bewertet. Dabei zeigten sich keine statistisch signifikanten Unterschiede zwischen den Studiengruppen. Aufgrund der Limitationen dieser sowie vergleichbarer Studien und angesichts uneinheitlicher Studienergebnisse bei insgesamt unzureichender Vergleichbarkeit der verschiedenen Akupunkturstudien ist eine abschließende Bewertung zur Wirksamkeit der Akupunktur nach Implantation einer Kniegelenks-Totalendoprothese jedoch nicht möglich. Dazu bedarf es weiterer, qualitativ hochwertiger und größerer Studien zu diesem Thema.
Es sollte eine dynamische MRT Methode entwickelt werden, welche die Berechnung und Darstellung einer Achse einer komplexen Bewegung ermöglicht. Dabei war die eigens auferlegte Definition von „Dynamik“, dass ein kompletter Bewegungsablauf als dreidimensionaler Datensatz im Zeitverlauf erfasst und ausgewertet werden soll (4 Dimensionen).
Diese Methode sollte auf die komplexe Bewegung des Kniegelenks angewendet werden und mit einem Knie-Modell verglichen werden.
Schlussendlich sollte die Flexion-Extensionsbewegung als vereinfacht monoaxial verifiziert werden.
Für diese Ziele wurden folgende Schritte durchgeführt:
Es wurden zunächst 10 Kadaver-Knie präpariert und hierbei u.a. kontrastmittelhaltige Kugeln als Tracker eingebracht. Die Knie wurden dann im Rahmen des Versuchsaufbaus in einer speziellen, pneumatischen Bewegungsschiene befestigt, welche dann automatisiert eine passive Bewegung vollzog. Diese wurde mit einer dynamischen MRT Methode festgehalten. Der entstandene 4D-Datensatz wurde eingelesen und die Kugelpositionen ausgewertet. Die Koordinaten der Positionen dienten dann als Grundlage für spezielle mathematische Algorithmen, welche die Flexion-Extensionsachse und ggf. die Innen- und Außenrotationsachse berechneten und schließlich eine graphische Darstellung ermöglichten. Zur Überprüfung wurde zudem ein Knie-Modell herangezogen und von diesem die gleichen Berechnungen durchgeführt.
Die Möglichkeiten der operativen Rekonstruktion degenerativ veränderter Hüftgelenke sind komplex und vielfältig. Bei den derzeit zur Verfügung stehenden operativen Behandlungsmassnahmen führen die Vor- und Nachteile immer wieder zur Diskussionen und Abwägung der Operationsverfahren. Hierbei stehen sich die rasche postoperative Mobilisierung sowie eine verminderte Rekonvaleszenzzeit mit den diskutierten Nachteilen einer schlechteren Übersichtlichkeit und damit verbundenen Fehlimplantationen gegenüber. Dies und die damit verbundene volkswirtschaftliche Bedeutung sind ein ständiger Ausgangspunkt für das Bemühen den optimalen Zugangsweg zu etablieren. Daher stellte das von Smith-Peterson 1949 publizierte Verfahren einen Meilenstein in der operativen Therapie dar. Hierdurch konnten zum einen die operationstechnischen Vorteile wie auch das volkswirtschaftliche Begehren nach kürzeren postoperativen Verweildauern vereint werden. Die Modifizierung dieses Zugangsweges hat sich bereits in einer großen Anzahl prospektiver Studien als zuverlässiges Rekonstruktionsverfahren etabliert und erfüllt zudem auch die Anforderungen der heutigen Medizin nach ästhetisch schönen Ergebnissen. In der vorliegenden Arbeit wurde eine prospektive Fallstudie des direkten anterioren Zugangs mit einem gesunden Vergleichskollektiv durchgeführt. Mit dem Ziel, die Aktivität ein Jahr postoperativ nach Implantation einer HTEP mit gesunden Probanden zu vergleichen. Von Januar 2009 bis Mai 2011 wurden insgesamt 77 Patienten und 59 Probanden in die Studie aufgenommen. Als Vergleichswerte wurde zum einen die klinische wie auch die radiologische Untersuchung herangezogen. In der klinischen Untersuchung zeigte sich insgesamt ein signifikanter Anstieg der untersuchten Scores im Vergleich mit den präoperativen Ergebnissen bei den Operierten. Im Vergleich zu den Probanden erzielen die Patienten ein Jahr nach HTEP teilweise noch schlechtere Werte in dem Bewegungsumfang und den Aktivitätsniveaus welche mittels der Auswertung des Stepwatches, des TWB und des Arzt-Patienten-Fragebogens erhoben wurden. Die radiologische Bewertung diente zur Feststellung der Positionierung der HTEP. Mit guten Positionierungen durch den direkten anterioren Zugang. Die Bewertung der Funktionalität zwischen den beiden Gruppen erfolgte durch den HHS, XSFMA- D und den Arzt-Patientenfragenbogen. Hierbei konnten ähnliche Ergebnisse, wie bereits oben beschrieben, verzeichnet werden mit guten Werten in der Gruppe der untersuchten Patienten, jedoch einer geringeren Funktionalität im Vergleich zu den Probanden. Die vorliegende Arbeit zeigt, dass der direkte anteriore Zugang die Wiederherstellung eines guten postoperativen Gesundheitszustandes mit erreichen eines hohen postoperativen Aktivitätslevels der Patienten ermöglicht. Ebenso erfüllt dieser Zugangsweg die Anforderungen der heutigen Medizin im Sinne einer schnellen postoperativen Mobilisation. Im Vergleich zu anderen minimal-invasiven Verfahren zeigen sich eine gute Implantierbarkeit, eine gute Positionierung und ein niedriges Komplikationsniveau. Prinzipiell hat der minimal-invasive anteriore Zugang das Potenzial sich als ein Standardverfahren in der operativen Rekonstruktion bei Hüftgelenksersatz zu etablieren, jedoch wäre ein direkter Vergleich mit dem lateralen Zugang erstrebenswert und sollte in weiteren Studien verglichen werden.
Purpose. Copal\(^®\) spacem is a new PMMA bone cement for fabricating spacers. This study compares elution of gentamicin, elution of vancomycin, and compressive strength of Copal\(^®\) spacem and of Palacos\(^®\) R+G at different vancomycin loadings in the powder of the cements. We hypothesized that antibiotic elution of Copal\(^®\) spacem is superior at comparable compressive strength. Methods. Compression test specimens were fabricated using Copal\(^®\) spacem manually loaded with 0.5 g gentamicin and additionally 2 g, 4 g, and 6 g of vancomycin per 40 g of cement powder (COP specimens) and using 0.5 g gentamicin premixed Palacos\(^®\) R+G manually loaded with 2 g, 4 g, and 6 g of vancomycin per 40 g of cement powder (PAL specimens). These specimens were used for determination of gentamicin and vancomycin elution (in fetal calf serum, at 22°C) and for determination of compressive strength both prior and following the elution tests. Results. Cumulative gentamicin concentrations (p < 0.005) and gentamicin concentration after 28 days (p ≤ 0.043) were significantly lower for COP specimens compared to PAL specimens. Cumulative vancomycin concentrations were significantly higher (p ≤ 0.043) for COP specimens after the second day. Vancomycin concentrations after 28 days were not significantly higher for the Copal specimens loaded with 2 g and 4 g of vancomycin. Compressive strength was not significantly different between COP specimens and PAL specimens before elution tests. Compressive strength after the elution tests was significantly lower (p = 0.005) for COP specimens loaded with 2 g of vancomycin. Conclusion. We could not demonstrate consistent superior antibiotic elution from Copal\(^®\) spacem compared to Palacos\(^®\) R+G for fabricating gentamicin and vancomycin loaded spacers. The results do not favor Copal\(^®\) spacem over Palacos\(^®\) R+G for the use as a gentamicin and vancomycin biantibiotic-loaded spacer.
Background:
We hypothesized, that periprosthetic joint infection (PJI) accounts for the major proportion of first (primary) and repeated (secondary) Total Knee Arthroplasty revisions at our university referral arthroplasty center.
Methods:
One thousand one hundred forty-three revisions, performed between 2008 and 2016 were grouped into primary (55%) and secondary (45%) revisions. The rate of revision indications was calculated and indications were categorized by time after index operation. The odds ratios of the indications for primary versus secondary revision were calculated.
Results:
In the primary revision group PJI accounted for 22.3%, instability for 20.0%, aseptic loosening for 14.9% and retropatellar arthrosis for 14.2%. PJI (25.6%) was the most common indication up to 1 year after implantation, retropatellar arthrosis (26.8%) 1–3 years and aseptic loosening (25.6%) more than 3 years after implantation. In the secondary revision group PJI accounted for 39.7%, aseptic loosening for 16.2% and instability for 13.2%. PJI was the most common indication at any time of revision with 43.8% up to one, 35.4% 1–3 years and 39.4% more the 3 years after index operation. The odds ratios in repeated revision were 2.32 times higher (p = 0.000) for PJI. For instability and retropatellar arthrosis the odds ratios were 0.60 times (p = 0.006) and 0.22 times (p = 0.000) lower.
Conclusions:
PJI is the most common indication for secondary TKA revision and within one year after primary TKA. Aseptical failures such as instability, retropatellar arthrosis and aseptical loosening are the predominant reasons for revision more than one year after primary TKA.
Purpose. Detection of infection persistence during the two-stage exchange of the knee for periprosthetic joint infection is challenging. Synovial fluid culture (SFC) and synovial white blood cell count (SWBCC) before joint reimplantation are widespread diagnostic means for this indication. The sensitivity and specificity of SFC and of SWBCC for infection persistence before planned reimplantation were evaluated. Methods. 94 two-stage exchanges of the knee with synovial fluid aspiration performed after a drug holiday of at least 14 days and before reimplantation or spacer exchange (planned reimplantation) were retrospectively analyzed. Only cases with at least 3 intraoperative samples at planned reimplantation were included. SFC and SWBCC were compared to pathogen detection (SFC\(_{(culture)}\)/SWBCC\(_{(culture)}\) and to histopathological signs of infection persistence (SFC\(_{(histo)}\)/SWBCC\(_{(histo)}\) from intraoperative samples at planned reimplantation. For SFC, the sensitivity and specificity were calculated. For SWBCC, the optimal cut-off value with its sensitivity and specificity was calculated with the Youden-Index. Results. Sensitivity and specificity of SFC\(_{(culture)}\) were 0.0% and 98.9%. Sensitivity and specificity of SFC\(_{(histo)}\) were 3.4% and 100%. The optimal cut-off value for SWBCC\(_{(culture)}\) was 4450 cells/μl with a sensitivity of 50.0% and a specificity of 86.5%. The optimal cut-off value for SWBCC\(_{(histo)}\) was 3250 cells/μl with a sensitivity of 35.7% and a specificity of 92.9%. Conclusion. The detection of infection persistence remains challenging and a consented approach is lacking. The results do not warrant the routine performance of SFC during the two-stage exchange at the knee. SWBCC can be used to confirm infection persistence at high cut-offs, but they only occur in few patients and are therefore inappropriate for the routine use.
Background:
The presented prospective randomized controlled single-centre study compares the clinical outcome up to 12 months after total hip arthroplasty using a minimally invasive single-incision direct anterior (DAA) and a direct transgluteal lateral approach.
Methods:
A total of 123 arthroplasties were evaluated utilizing the Harris Hip Score (HHS), the extra short musculoskeletal functional assessment questionnaire (XSFMA), the Short Form 36 (SF-36) health survey, a Stepwatch™ Activity Monitor (SAM), and a timed 25 m foot walk (T25-FW). Postoperative x-ray images after THA were reviewed to determine inclination and stem positioning.
Results:
At final follow-up, the XSFMA functional index scores were 10.3 (anterior) and 15.08 (lateral) while the bother index summed up to a score of 15.8 (anterior) and 21.66 (lateral) respectively, thus only differing significantly for the functional index (p = 0.040 and p = 0.056). The SF-36 physical component score (PCS) was 47.49 (anterior) and 42.91 (lateral) while the mental component score (MCS) summed up to 55.0 (anterior) and 56.23 (lateral) with a significant difference evident for the PCS (p = 0.017; p = 0.714). Patients undergoing THA through a DAA undertook a mean of 6402 cycles per day while those who had undergone THA through a transgluteal approach undertook a mean of 5340 cycles per day (p = 0.012). Furthermore, the obtained outcome for the T25-FW with 18.4 s (anterior) and 19.75 s (lateral) and the maximum walking distance (5932 m and 5125 m) differed significantly (p = 0.046 and p = 0.045). The average HHS showed no significant difference equaling 92.4 points in the anterior group and 91.43 in the lateral group (p = 0.477). The radiographic analysis revealed an average cup inclination of 38.6° (anterior) and 40.28° (lateral) without signs of migration.
Conclusion:
In summary, our outcomes show that after 1 year THA through the direct anterior approach results in a higher patient activity compared to THA utilizing a transgluteal lateral approach while no differences regarding hip function are evident.

Background:
Evaluation of infection persistence during the two-stage exchange of the hip is challenging. Joint aspiration before reconstruction is supposed to rule out infection persistence. Sensitivity and specificity of synovial fluid culture and synovial leucocyte count for detecting infection persistence during the two-stage exchange of the hip were evaluated.
Methods:
Ninety-two aspirations before planned joint reconstruction during the two-stage exchange with spacers of the hip were retrospectively analyzed.

The sensitivity and specificity of synovial fluid culture was 4.6 and 94.3%. The sensitivity and specificity of synovial leucocyte count at a cut-off value of 2000 cells/μl was 25.0 and 96.9%. C-reactive protein (CRP) and erythrocyte sedimentation rate (ESR) values were significantly higher before prosthesis removal and reconstruction or spacer exchange (p = 0.00; p = 0.013 and p = 0.039; p = 0.002) in the infection persistence group. Receiver operating characteristic area under the curve values before prosthesis removal and reconstruction or spacer exchange for ESR were lower (0.516 and 0.635) than for CRP (0.720 and 0.671).
Conclusions:
Synovial fluid culture and leucocyte count cannot rule out infection persistence during the two-stage exchange of the hip.
Bone metastasis is a frequent and life-threatening complication of breast cancer. The molecular mechanisms supporting the establishment of breast cancer cells in the skeleton are still not fully understood, which may be attributed to the lack of suitable models that interrogate interactions between human breast cancer cells and the bone microenvironment. Although it is well-known that integrins mediate adhesion of malignant cells to bone extracellular matrix, their role during bone colonization remains unclear. Here, the role of β1 integrins in bone colonization was investigated using tissue-engineered humanized in vitro and in vivo bone models. In vitro, bone-metastatic breast cancer cells with suppressed integrin β1 expression showed reduced attachment, spreading, and migration within human bone matrix compared to control cells. Cell proliferation in vitro was not affected by β1 integrin knockdown, yet tumor growth in vivo within humanized bone microenvironments was significantly inhibited upon β1 integrin suppression, as revealed by quantitative in/ex vivo fluorescence imaging and histological analysis. Tumor cells invaded bone marrow spaces in the humanized bone and formed osteolytic lesions; osteoclastic bone resorption was, however, not reduced by β1 integrin knockdown. Taken together, we demonstrate that β1 integrins have a pivotal role in bone colonization using unique tissue-engineered humanized bone models.
Durch das konkav gekrümmte mediale Tibiaplateau und das konvex gekrümmte laterale Tibiaplateau, welches zudem etwas nach dorsal versetzt ist, entsteht ein mechanisches Viergelenk. Dieses Viergelenk erzwingt im menschlichen Knie eine reibungsarme Roll-Gleitbewegung. Handelsübliche Knieendoprothesen haben dieser anatomischen Besonderheit bis dato keine Rechnung getragen.
In der seit 2003 eingeführten neuartigen Knieprothese „AEQUOS G1“ wird diese anatomische Besonderheit erstmalig berücksichtigt. Bei Belastung ist die Rollbewegung die dominierende Bewegungsform und minimiert so den Materialverschleiß des Polyethylen-Inlays. Durch eine dominierende Gleitbewegung bei Beugewinkeln > 60° ermöglich diese Prothese zugleich eine maximale Flexion.
Diese Arbeite beschäftigt sich mit dem klinischen Outcome und der Patientenzufriedenheit nach Implantation einer Aequos G1 Prothese in einer mittelfristigen Follow-up Studie. Die Frage, ob sich die ersten vielversprechenden Ergebnisse reproduzieren lassen, ist ebenfalls Gegenstand der vorliegend Arbeit.
Es wurde bei 54 Patienten eine Implantation einer Aequos G1 Prothese durchgeführt, diese wurden in einer Monocenterstudie nachuntersucht.
Die Patienten wurden nach mindestens 24 monatiger Prothesenstandzeit klinisch untersucht und mit Hilfe verschiedener Fragebögen Interviewt. Als Fragebögen wurden verwendet: der Kujala-Score (KS), der Turba-Score (TS), der Knee Society Score (KSS), die visuelle Analogskala Schmerz (VAS), der Oxford Knee Score (OKS) und der Short-Form-36 (SF-36).
Es wurden 46 Patienten nach durchschnittlich 40,89 Monaten nachuntersucht. Das Durchschnittsalter belief sich auf 70,30 Jahre, bei einem Frauenanteil von 73,90 % und folglich einem Männeranteil von 26,10 %. Der durchschnittliche BMI konnte mit 32,71 kg/m² bestimmt werden.
Das mittlere Bewegungsausmaß betrug in der vorliegenden Studie 108,48° ± 9,65°. Dieses Ergebnis zeigt sich nahezu identisch mit den Ergebnissen der kurzfristigen Vergleichsstudien von Frosch und Will. [15, 67]
In der Revisionsendoprothetik des Hüftgelenks bietet ein modulares Schaftsystem die Möglichkeit der intraoperativen Anpassung an die individuellen anatomischen Gegebenheiten. Dies ist eine Untersuchung des klinisch-radiologischen Outcomes des MRP-Revisionsschafts mit diaphysärer Fixation in der Revisionschirurgie. Der MRP-Revisionsschaft zeigt ein zufriedenstellendes klinisches Outcome.
The role of serum amyloid A (SAA) proteins, which are ligands for toll-like receptors, was analyzed in human bone marrow-derived mesenchymal stem cells (hMSCs) and their osteogenic offspring with a focus on senescence, differentiation andmineralization. In vitro aged hMSC developed a senescence-associated secretory phenotype (SASP), resulting in enhanced SAA1/2, TLR2/4 and proinflammatory cytokine (IL6, IL8, IL1\(\beta\), CXCL1, CXCL2) expression before entering replicative senescence. Recombinant human SAA1 (rhSAA1) induced SASP-related genes and proteins in MSC, which could be abolished by cotreatment with the TLR4-inhibitor CLI-095. The same pattern of SASP-resembling genes was stimulated upon induction of osteogenic differentiation, which is accompanied by autocrine SAA1/2 expression. In this context additional rhSAA1 enhanced the SASP-like phenotype, accelerated the proinflammatory phase of osteogenic differentiation and enhanced mineralization. Autocrine/paracrine and rhSAA1 via TLR4 stimulate a proinflammatory phenotype that is both part of the early phase of osteogenic differentiation and the development of senescence. This signaling cascade is tightly involved in bone formation and mineralization, but may also propagate pathological extraosseous calcification conditions such as calcifying inflammation and atherosclerosis.
