Background
The epidural route is still considered the gold standard for labour analgesia, although it is not without serious consequences when incorrect placement goes unrecognized, e.g. in case of intravascular, intrathecal and subdural placements. Until now there has not been a viable alternative to epidural analgesia especially in view of the neonatal outcome and the need for respiratory support when long-acting opioids are used via the parenteral route. Pethidine and meptazinol are far from ideal having been described as providing rather sedation than analgesia, affecting the cardiotocograph (CTG), causing fetal acidosis and having active metabolites with prolonged half-lives especially in the neonate. Despite these obvious shortcomings, intramuscular and intravenously administered pethidine and comparable substances are still frequently used in delivery units.
Since the end of the 90ths remifentanil administered in a patient-controlled mode (PCA) had been reported as a useful alternative for labour analgesia in those women who either don’t want, can’t have or don’t need epidural analgesia.
Discussion
In view of the need for conversion to central neuraxial blocks and the analgesic effect remifentanil has been demonstrated to be superior to pethidine. Despite being less effective in terms of the resulting pain scores, clinical studies suggest that the satisfaction with analgesia may be comparable to that obtained with epidural analgesia. Owing to this fact, remifentanil has gained a place in modern labour analgesia in many institutions.
However, the fact that remifentanil may cause harm should not be forgotten when the use of this potent mu-agonist is considered for the use in labouring women. In the setting of one-to-one midwifery care, appropriate monitoring and providing that enough experience exists with this potent opioid and the treatment of potential complications, remifentanil PCA is a useful option in addition to epidural analgesia and other central neuraxial blocks. Already described serious consequences should remind us not refer to remifentanil PCA as a “poor man’s epidural” and to safely administer remifentanil with an appropriate indication.
Summary
Therefore, the authors conclude that economic considerations and potential cost-savings in conjunction with remifentanil PCA may not be appropriate main endpoints when studying this valuable method for labour analgesia.
Nachuntersuchung extrem kleiner Frühgeborener mit der Bayley Mental Scale im Alter von drei Jahren

Nach der Bundesgeschäftsstelle für Qualitätssicherung (BQS) hatten in Deutschland 2007/2008 0,7% aller Kinder ein Geburtsgewicht unter 1000 g. Die Kinder unserer Studie kamen in den Jahren 2001 und 2002 in der Frauen-klinik der Universität Würzburg zur Welt und wogen bei der Geburt (GG) unter 1000 g und werden somit als extrem kleine Frühgeborene bezeichnet (ELBW). Diese 24 Kinder wurden bis zu einem Alter von korrigierten 42 Monaten mit den Bayley Scales of Infant Development II untersucht, um global ihre kognitive Ent-wicklung einschätzen zu können. Zeitgleich befasste sich eine Studie (HAGELAUER 2009) mit Kindern der gleichen Jahrgänge, die ein GG von 1000 bis 1500 g aufwiesen (VLBW). Weiterhin wurde ein Kollektiv von termingeborenen, gesunden Kindern ein-bezogen, sodass ein Vergleich der Leistung aller 3 Gruppen vorgenommen werden konnte. Die insgesamt 24 ELBW erzielten einen Mental Developmental Index (MDI) von 88,3, was bei einem Normbereich von 85 bis 115 einem gerade noch durch-schnittlichen Ergebnis entspricht. In 2 Fällen konnte die Untersuchung nicht be-endet werden, und einmal war das Kind von vornherein nicht dazu in der Lage. Bei diesen 3 Frühgeborenen und bei 8 weiteren dieser Gruppe, die eine Leistung unterhalb der Norm erbrachten, wurden zusätzlich, anhand der Krankengeschichte, mögliche Gründe für das Abschneiden ermittelt. Dabei stellte sich heraus, dass die Summe mehrerer, in der Literatur häufig be-schriebener, Risikofaktoren und Diagnosen mit dem Ergebnis in Zusammen-hang standen. Es stellten sich signifikante Unterschiede in der Leistung der Kinder mit und ohne intrazerebraler Blutung heraus. Ebenso schnitten Kinder, deren Mütter eine höheren Schulabschluss hatten, signifikant besser ab. Lediglich tendenziell bessere Ergebnisse erzielten Mädchen, eutrophe Kinder, Frühgeborene mit für ihr Alter normalem Kopfumfang, Kinder mit normalem pH-Wert der Nabelschnurarterie und Frühgeborene, deren Väter einen höheren Schulabschluss hatten. Die 28 VLBW der parallelen Studie erreichten 102,5 Punkte, also einen Wert, der recht genau dem Durchschnitt entspricht. Auch in dieser Gruppe konnte in 2 Fällen kein MDI ermittelt werden. Die Kontrollgruppe mit 32 gesunden Termingeborenen war mit 111,6 Punkten beinahe überdurchschnittlich, ohne dass ein Kind < 85 Punkte erhielt. Der Grund für die Zusammenstellung einer Kontrollgruppe war die Normierung der BSID II auf amerikanische Kinder. Mit den gesunden Kindern konnten wir einschätzen, wie heimische, reife Gleichaltrige den Test meistern. Statistisch errechnete sich ein signifikanter Unterschied zwischen der Leistung der ELBW im Vergleich zur Kontrollgruppe. Gleiches gilt für die VLBW. Allerdings bestand keine signifikante Abweichung in den Ergebnissen der extrem und sehr kleinen Frühgeborenen. Weiter wurde der Umgang mit Frühgeborenen an der Grenze zur Lebensfähig-keit und die spätere Lebensqualität der Überlebenden beleuchtet. Hier sind sich fast alle Autoren einig, dass auf der Basis von Leitlinien individuelle Ent-scheidungen aller Beteiligten getroffen werden müssen, und dass sich die oft eingeschränkte Lebensqualität mit Hilfe von Fördermaßnahmen verbessern lässt. Der hohe Stellenwert der BSID II in der Entwicklungsdiagnostik in Deutschland ist einerseits durch die guten Eigenschaften des Tests bedingt, andererseits aber auch durch den Beschluss des Gemeinsamen Bundesausschusses, der die Untersuchung aller Frühgeborenen mit einem Geburtsgewicht unter 1500 g bis zu einem Alter von 2 Jahren fordert. Allerdings sind zur Beurteilung der Langzeitentwicklung und Lebensqualität auch Follow-up Studien im späteren Kindesalter, bei Jugendlichen und bei Er-wachsenen nötig, die international immer noch rar sind. Konzepte hierfür sind, neben der Verbesserung der Behandlungsqualität Frühgeborener, Gegenstand der aktuellen Forschung.
Der transkriptionelle Repressor Blimp-1 wurde ursprünglich als essentiell für die terminale Differenzierung von B-Zellen zu Antikörper-produzierenden Plasmazellen beschrieben. Im Rahmen dieser Arbeit wurde die Expression von Blimp-1 in humanen T-Zellen untersucht. Die Versuchsergebnisse zeigen, dass Blimp-1 auch in humanen T-Zellen sowohl auf mRNA- als auch auf Proteinebene exprimiert wird. Naproxen-induzierte Pseudoporphyrie bei Kindern mit Erkrankungen des rheumatischen Formenkreises

Bei der Naproxen-induzierten Pseudoporphyrie bei Kindern und Jugendlichen mit Erkrankungen des rheumatischen Formenkreises handelt es sich um ein Krankheitsbild, das durch Hauterscheinungen charakterisiert wird, die klinisch und histologisch nicht von einer Porphyria cutanea tarda zu unterscheiden sind. Es zeigen sich Bläschen, Blasen und eine erhöhte Verletzlichkeit der Haut in sonnenexponierten Arealen. Im Gegensatz zur Porphyria cutanea tarda lassen sich keine pathologischen Porphyrinstoffwechselstörungen nachweisen 159-176. Neben vielen anderen Medikamenten wird Naproxen als häufiger Auslöser einer Pseudoporphyrie beschrieben. Kinder und Jugendliche, die aufgrund einer rheumatischen Grunderkrankung eine Naproxentherapie erhalten, sind gefährdet, eine Pseudoporphyrie zu entwickeln. In der vorliegenden Untersuchung wurden Daten von insgesamt 196 Patienten erfasst, von denen 45 an einer Pseudoporphyrie unter einer Naproxentherapie erkrankt waren, 96 eine Naproxentherapie erhielten, ohne eine Pseudoporphyrie zu entwickeln und 55 keine Therapie mit Naproxen erhalten hatten. Die Inzidenz einer Pseudoporphyrie betrug 11,4% bei Patienten, die eine Naproxentherapie erhalten hatten. In der Untersuchung fiel auf, dass die Diagnose der EOPA-JIA bei Patienten mit einer Pseudoporphyrie signifikant häufiger zugrunde lag, als bei Patienten, die keine Pseudoporphyrie entwickelten. Eine Geschlechtsbezogenheit beim Auftreten einer Pseudoporphyrie konnte im Vergleich mit den beiden anderen Patientengruppen nicht gefunden werden, dennoch fiel auf, dass drei Viertel der Pseudoporphyrie-Patienten weiblichen Geschlechts waren. Patienten, die eine Pseudoporphyrie entwickelten, waren beim Auftreten der rheumatischen Grunderkrankung signifikant jünger, als die Patienten der beiden anderen Gruppen. Ein Zusammenhang von Auftreten einer Pseudoporphyrie und Laborparametern konnte bei folgenden Werten gefunden werden: niedriges Hämoglobin, erhöhte Leukozytenzahlen, erhöhte Thrombozytenzahlen und Vorhandensein von Eiweiß im Urin. Patienten mit einer insgesamt höheren Krankheitsaktivität der rheumatischen Grunderkrankung, erhöhten Entzündungszeichen und einem erniedrigten Hämoglobin-Wert haben ein erhöhtes Risiko, eine Pseudoporphyrie zu entwickeln. Pseudoporphyrie-Patienten hatten signifikant häufiger einen hellen Hauttyp. Hinsichtlich Augen- und Haarfarbe, sowie dem Auftreten von Sommersprossen fanden sich jedoch keine signifikanten Unterschiede, ebenso wenig hinsichtlich der Hautpflegemaßnahmen, sowie des Auftretens von Psoriasis oder Atopieneigung beim Patienten oder erstgradigen Verwandten. Patienten, die an einer Pseudoporphyrie erkrankten, waren im Sommer und im Urlaub signifikant häufiger einer erhöhten Sonnenexposition ausgesetzt. Eine Dosisabhängigkeit bezüglich des Auftretens einer Pseudoporphyrie ließ sich nachweisen. Der Erkrankungsgipfel fand sich im Alter von acht Jahren, vier Fünftel der Pseudoporphyrie-Patienten erkrankten innerhalb der ersten zwei Jahre der Naproxentherapie, in der untersuchten Gruppe fanden sich in 15% schwere, in 46% mittelschwere und in 38% leichte Verläufe des Krankheitsbildes. In der vorliegenden Untersuchung konnte erstmalig ein Zusammenhang zwischen einer erhöhten entzündlichen Aktivität und dem Auftreten einer Pseudoporphyrie gezeigt werden. Daraus ergeben sich neue Forschungsansätze, die in weiteren Untersuchungen geklärt werden könnten.
Background
Incontinentia Pigmenti is a rare disease affecting multiple organs. Fifty of patients show affection of the eye with retinopathy and possible amaurosis being the worst outcome. Treatment has commonly been panretinal laser coagulation but intravitreal application of bevacizumab as VEGF-inhibitor has shown to effectively suppress retinal neovascularization.
Case presentation
A six-week-old female infant with Incontinentia Pigmenti developed a foudroyant necrotizing enterocolitis shortly after intravitreal injection of bevazicumab due to a retinopathy with impending tractional detachment of the left eye. Since the onset of abdominal symptoms occurred immediately after the intravitreal application, a link between the two events seemed likely. Sequential analyses of the VEGF serum concentrations showed a massive suppression of endogenous VEGF with only a very slow recovery over weeks. Such a severe systemic adverse event has not been reported after intravitreal treatment with bevacizumab in an infant.
Conclusion
This case report shows a relevant systemic uptake of bevacizumab after intravitreal application as suppressed VEGF levels show. There seems to be a connection between suppressed VEGF levels and the onset of necrotizing enterocolitis. Therefore, treatment with bevacizumab should be carefully considered and further research is needed to assess this drug’s safety profile.
Das Medulloblastom ist der häufigste, maligne Hirntumor des Kindesalters. In den letzten Jahren ist es gelungen, molekularbiologisch definierte Subgruppen innerhalb dieser Tumorentität zu definieren, die sich durch ein unterschiedliches therapeutisches Ansprechen differenzieren lassen. Es werden vier Gruppen abgegrenzt: Tumore mit Aktivierung des WNT-Signalwegs, des SHH-Signalwegs, sowie der Gruppe 3, die sich vor allem durch eine Myc Amplifikation auszeichnen und Gruppe 4, in der MycN amplifiziert wird. Während Patienten mit SHH- und WNT-Tumoren eher eine günstige Prognose haben, ist der Bedarf an neuen Therapieansätzen für Patienten mit Gruppe 3- und Gruppe 4-Tumoren auf Grund der schlechten Prognose sehr hoch. Die aus einem malignen Pleuraerguss eines Gruppe 3-Medulloblastom Patienten gewonnenen Tumorzellen konnten erfolgreich als permanente Zelllinie etabliert sowie in vitro und in vivo charakterisiert werden. Dieses Tumormodell habe ich in meiner Dissertationsarbeit genutzt um die zytotoxische Wirkung neuer Zytostatika und Inhibitoren Kombinationen auf das Gruppe 3-Medulloblastom in vitro und im Tiermodell zu untersuchen. Bei der Auswahl neuer Therapieansätze galt die Maßgabe, Substanzen zu wählen, die für den klinischen Einsatz geeignet sind, neue gezielte zytostatische Mechanismen aufweisen und bei denen bereits publizierte Vorarbeiten nahelegen, dass diese das ZNS tatsächlich erreichen. In unserem in vitro Modell testeten wir 23 Substanzen und wählten dann die fünf vielversprechendsten für das subkutane Tumormausmodell aus. Als Vergleichsgruppen für das Therapieansprechen wählten wir die Kombination aus Cisplatin und Etoposid, die gerade bei den Patienten mit einem HochrisikoMedulloblastom die Therapie der Wahl ist. Im ersten Tierversuch konnte gezeigt werden, dass Gemcitabin als Monotherapie den gleichen zytotoxischen Therapieeffekt zeigt, wie Cisplatin und Etoposid in Kombination. Deshalb war nun das Ziel eine Substanz zu finden, welche dann in Kombination mit Gemcitabin einen besseres Therapieansprechen als die Standardtherapie zeigt. Bei der Tumorpräparation im initialen Tierversuch fiel auf, dass die Tumoren stark vaskularisiert sind. Nach dieser Beobachtung stellten wir uns die Fragen, ob die Inhibition des Vaskularisierungsvorgangs im Tumor einen additiven zytotoxischen Effekt auf das Tumorwachstum hat. Daraufhin erweiterten wir die Suche nach geeigneten Substanzen für das Gruppe 3-Medulloblastom um Multikinaseinhibitoren, die auch VEGF-Rezeptoren inhibieren. Überraschenderweise konnten wir zeigen das Gemcitabin in Kombination mit verschiedenen VEGF-Rezeptor-Inhibitoren in den Toxizitätsversuchen in vitro eine 10.000 fach niedrigere EC50 hat als die Kombination aus Cisplatin und Etoposid. Auch in den anschließenden Tierversuchen konnten wir zeigen, dass Gemcitabin in Kombination mit Axitinib einen besseren Therapieerfolg bezogen auf das Tumorwachstum zeigt, als die bisherige Standardtherapie. Darüber hinaus verloren die Tiere, die mit Gemcitabin und Axitinib behandelt wurden deutlich weniger an Gewicht, als die Tiere die die Standardtherapie erhielten. Zusammenfassend können wir zeigen, dass Axitinib, ein neuer pan-VEGF-Rezeptor-Inhibitor der bisher nur in anderen Tumorentitäten wie dem Nierenzellkarzinom zum Einsatz kommt, in Kombination mit Gemcitabin, einem Nukleosidanalogon, einen deutlichen zytotoxischen Effekt an Medulloblastom Zelllinien in vitro als auch im subkutanen und orthotopen Tiermodell hat. Diese Ergebnisse könnten die Basis sein für neue therapeutische Strategien gegen das Gruppe 3-Medulloblastom bei Kindern, die die bisherige Therapie nicht mehr tolerieren oder bereits irreversible Nebenwirkungen der Standardtherapie entwickelt haben
Die gesetzlich vorgeschriebene Nachsorge von Frühgeborenen in Deutschland beschränkt sich nach den Vorgaben des G-BA momentan auf eine Entwicklungstestung mit den Bayley Scales of Infant Development im Alter von zwei Jahren. Entwicklungsuntersuchungen zu einem späteren Zeitpunkt sind jedoch notwendig, da neurologische Folgen bzw. Auswirkungen dann besser beurteilt und gemessen werden können.
Die WUEP-KD ist eine neuropsychologische Testbatterie, die auf der CHC-Theorie basiert und den Vorteil einer guten Normierung und Validierung für deutsche Kinder, sowie einer kurzen Durchführungszeit hat. Außerdem wurden bereits langjährig Erfahrungen in der Anwendung bei Kindern mit anderen neuropsychologischen Problemen gesammelt.
Wir wendeten die WUEP-KD bei sechs bis acht Jahre alten Kindern an, die in den Jahren 2001 und 2002 in der Frauenklinik der Universität Würzburg mit einem Geburtsgewicht von unter 1500g zur Welt gekommen waren und in der Universitätskinderklinik Würzburg behandelt wurden. Weiterhin wurden zehn termingerecht geborene und gesunde Kinder im gleichen Alter untersucht.
Es stellte sich heraus, dass die Frühgeborenen, die an unserer Studie teilgenommen hatten, signifikant besser bei den BSID-II im Alter von zwei Jahren abgeschnitten hatten als diejenigen, die wir leider nicht von einer Teilnahme überzeugen konnten. Tendenziell zeigte sich in unserer Studie bezüglich der zentralen mentalen Leistungsfähigkeit dennoch eine geringere Leistung bei geringerem Gestationsalter und bzw. oder geringerem Geburtsgewicht. Die Ergebnisse des Untertests CPM, welcher die fluide Intelligenz abbildet, waren signifikant unterschiedlich beim Gruppenvergleich der Geburtsgewichte sowie des Gestationsalters. Somit konnten wir mit unserer Methodik ebenso wie in anderen Studien einen Unterschied in der kognitiven Leistung zwischen den VLBW-Kindern und den ELBW-Kindern im Alter von sechs bis acht Jahren nachweisen. Beim Vergleich mit den Untersuchungen im Alter von zwei bis drei Jahren konnten wir weitgehend eine gleichbleibende Leistung nachweisen, die Ergebnisse der Bayley-Scales und der mentalen Gesamtleistung der WUEP-KD korrelierten signifikant.
Um eine umfassende Diagnostik durchzuführen und weitere Intelligenzfaktoren nach der CHC-Theorie zu erfassen, werden in der WUEP-KD computerisierte Tests verwendet. Zur Messung der feinmotorischen Leistung wurde hierfür das Speed-Tapping verwendet, welches bisher nicht in der Untersuchung Frühgeborener angewandt wurde. Die feinmotorischen Fähigkeiten der Früh- und Reifgeborenen lagen durchschnittlich im Normbereich, jedoch hatten doppelt so viele Frühgeborene als Reifgeborene Defizite in der Feinmotorik. Insbesondere die ELBW-Kinder waren hiervon betroffen. Bei Betrachtung der Frühgeborenen konnte eine signifikante Korrelation zwischen dem Gestationsalter und der feinmotorischen Leistung nachgewiesen werden. Somit konnten wir nachweisen, dass ein geringeres Geburtsgewicht und Gestationsalter das Risiko erhöhen, feinmotorische Defizite im Schulalter nachweisen zu können – auch wenn keine höhergradigen intrakraniellen Blutungen im Neugeborenenalter aufgetreten waren und die kognitive Leistung zum Zeitpunkt der Untersuchung im Normbereich liegt. Die WUEP-KD kann zusätzlich im Bereich der motorischen Fähigkeiten Defizite aufdecken.
Die Aufmerksamkeitsleistung, gemessen mit dem CPT, lag im Normbereich, dennoch waren wiederum vermehrt Defizite bei den ELBW-Kindern und den Kindern mit einem Gestationsalter unter 29 SSW zu beobachten.
Um das Verhalten und die Lebensqualität der frühgeborenen Kinder einschätzen zu können, ließen wir die Eltern drei Fragebögen beantworten (CBCL, SDQ, KINDL-R). Hier konnten wir größtenteils keine signifikanten Unterschiede zwischen den Reif- und Frühgeborenen feststellen. Im Fragebogen zur Lebensqualität konnten bei den Frühgeborenen sogar signifikant bessere Ergebnisse in den Bereichen „Freunde“ und „Selbstwert“ nachgewiesen werden.
Die WUEP-KD stellt aus unserer Sicht eine geeignete Methodik dar, um frühgeborene Kinder in ihrer weiteren Entwicklung nachzuuntersuchen – sie basiert auf der CHC-Theorie, dem Goldstandard der Intelligenzdiagnostik, hat eine kurze Durchführungsdauer, es besteht eine langjährige Anwendung und Erfahrung in der Durchführung bei Kindern mit neuropsychologischer Problematik und hat die nun nachgewiesene Fähigkeit kognitive und motorische Defizite bei frühgeborenen Kindern aufzudecken. Hierdurch können die betroffenen Kinder in ihren Fähigkeiten und Grenzen besser eingeschätzt und somit gezielt betreut werden.
Purpose of Review
To describe in detail the clinical synopsis and pathophysiology of chronic non-bacterial osteomyelitis and SAPHO syndrome.
Recent Findings
Chronic non-bacterial osteomyelitis (CNO) has been identified as a disease entity for almost 50 years. This inflammatory bone disorder is characterized by osteolytic as well as hyperostotic/osteosclerotic lesions. It is chronic in nature, but it can present with episodic flairs and phases of remission, which have led to the denomination “chronic recurrent osteomyelitis”, with its severe multifocal form “chronic recurrent multifocal osteomyelitis” (CRMO). For almost three decades, an infectious aetiology had been considered, since especially Propionibacterium acnes had been isolated from bone lesions of individual patients. However, this concept has been challenged since long-term antibiotic therapy did not alter the course of disease and modern microbiological techniques (including PCR) failed to confirm bone infection as an underlying cause. Over recent years, a profound dysregulation of cytokine expression profiles has been demonstrated in innate immune cells of CNO patients. A hallmark of monocytes from CNO patients is the failure to produce immune regulatory cytokines interleukin-10 (IL-10) and IL-19, which have been linked with genetic and epigenetic alterations. Subsequently, a significant upregulation of pro-inflammatory, NLRP3 inflammasome-dependent cytokines (IL-1β and TNF-α), has been demonstrated.
Summary
The current knowledge on CNO, the underlying molecular pathophysiology, and modern imaging strategies are summarized; differential diagnoses, treatment options, outcome measures, as well as quality of life studies are discussed.
Background: IL-12p40 plays an important role in the activation of the T-cell lines like Th17 and Th1-cells. Theses cells are crucial in the pathogenesis of juvenile idiopathic arthritis. A polymorphism in its promoter region and the genotype IL12p40 pro1.1 leads to a higher production of IL-12p40. We studied whether there is a difference in the distribution of the genotype in patients with JIA and the healthy population.
Methods: In 883 patients and 321 healthy controls the IL-12p40 promoter genotype was identified by ARMS-PCR.
Results: There is no association of IL-12p40 pro polymorphism neither in patients with JIA compared to controls nor in subtypes of JIA compared to oligoarthritis. We found a non-significant tendency of a higher prevalence of the genotype pro1.1 in systemic arthritis (32.4 %) and in rheumatoid factor negative polyarthritis (30.5 %) and a lower pro1.1 genotype in persistent oligoarthritis (20.7 %) and in enthesitis-related arthritis (17 %). Likelihood of the occurrence of genotype IL12-p40 pro1.1 in patients with systemic arthritis (OR 1.722, CI 95 % 1.344-2.615, p 0.0129) and RF-negative polyarthritis (OR 1.576, CI 95 % 1.046-2.376, p 0.0367) compared to persistent oligoarthritis was significantly higher. This was also true for comparison of their homozygous genotypes IL-12p40 pro 1.1 and 2.2 in systemic arthritis (OR 1.779, CI 95 % 1.045-3.029, p 0.0338). However, in Bonferroni correction for multiple hypothesis this was not significant.
Conclusion: A tendency of a higher prevalence of the genotype IL-12p40 pro1.1 in systemic arthritis and in rheumatoid factor negative polyarthritis was observed but not significant. Further investigations should be done to clarify the role IL-12p40 in the different subtypes of JIA.
Background:
Atypical chemokine receptor 3 (ACKR3, synonym CXCR7) is increasingly considered relevant in neuroinflammatory conditions, in which its upregulation contributes to compromised endothelial barrier function and may ultimately allow inflammatory brain injury. While an impact of ACKR3 has been recognized in several neurological autoimmune diseases, neuroinflammation may also result from infectious agents, including Ureaplasma species (spp.). Although commonly regarded as commensals of the adult urogenital tract, Ureaplasma spp. may cause invasive infections in immunocompromised adults as well as in neonates and appear to be relevant pathogens in neonatal meningitis. Nonetheless, clinical and in vitro data on Ureaplasma-induced inflammation are scarce.
Methods:
We established a cell culture model of Ureaplasma meningitis, aiming to analyze ACKR3 variances as a possible pathomechanism in Ureaplasma-associated neuroinflammation. Non-immortalized human brain microvascular endothelial cells (HBMEC) were exposed to bacterial lipopolysaccharide (LPS) or tumor necrosis factor-α (TNF-α), and native as well as LPS-primed HBMEC were cultured with Ureaplasma urealyticum serovar 8 (Uu8) and U. parvum serovar 3 (Up3). ACKR3 responses were assessed via qRT-PCR, RNA sequencing, flow cytometry, and immunocytochemistry.
Results:
LPS, TNF-α, and Ureaplasma spp. influenced ACKR3 expression in HBMEC. LPS and TNF-α significantly induced ACKR3 mRNA expression (p < 0.001, vs. control), whereas Ureaplasma spp. enhanced ACKR3 protein expression in HBMEC (p < 0.01, vs. broth control). Co-stimulation with LPS and either Ureaplasma isolate intensified ACKR3 responses (p < 0.05, vs. LPS). Furthermore, stimulation wielded a differential influence on the receptor’s ligands.
Conclusions:
We introduce an in vitro model of Ureaplasma meningitis. We are able to demonstrate a pro-inflammatory capacity of Ureaplasma spp. in native and, even more so, in LPS-primed HBMEC, underlining their clinical relevance particularly in a setting of co-infection. Furthermore, our data may indicate a novel role for ACKR3, with an impact not limited to auto-inflammatory diseases, but extending to infection-related neuroinflammation as well. AKCR3-induced blood-brain barrier breakdown might constitute a potential common pathomechanism.
